    Background Performance measures for brief alcohol interventions (BIs) are currently based on provider documentation of BI. However, provider documentation may not be a reliable measure of whether or not patients are offered clinically meaningful BIs. In particular, BI documented with clinical decision support in an electronic medical record (EMR) could appear identical irrespective of the quality of BI provided. We hypothesized that differences in how BI was implemented across health systems could lead to differences in the proportion of documented BI recalled and reported by patients across health systems. Methods Male outpatients with unhealthy alcohol use identified by confidential satisfaction surveys (2009–2012) were assessed for whether they reported receiving BI in the past year (patient-reported BI) and whether they had BI documented in the EMR during the same period (documented BI). We evaluated and compared the prevalence of documented BI to patient-reported BI across 21 VA networks to determine whether documented BI had a variable association with patient-reported BI across the networks. Results Of 9896 eligible male outpatients with unhealthy alcohol use, 59.0% (95% CI 57.4–60.5%) reported BI (50.4–64.9% across networks) and 37.4% (95% CI 36.0–38.9%) had BI documented in the EMR (28.0–44.2% across networks). Overall, 72.9% (95% CI 70.8–75.5%) of patients with documented BI also reported BI. The association between documented BI and patient-reported BI did not vary across VA networks in adjusted logistic regression models. Conclusions Performance measures of BI that rely on provider documentation in EMRs appear comparable to patient report for comparing care across VA networks. PMID:26072218

    A review of the archive's documents on the main stages of the creation of the synchrocyclotron at the Hydrotechnical Laboratory (now known as the Dzhelepov Laboratory of Nuclear Problems) is presented. The review is based on minutes of the Special Committee and Scientific and Technical Council (NTS), which belongs to the First Chief Directorate (PGU) concerning the era of the Soviet atomic project (1945-1953). The analysis of these minutes has enabled us to prepare a chronicle of events that deal with the creation of the synchrocyclotron, as well as the discussion of the project and its place of construction, the research and development program, and so on. The minutes of two NTS meetings dated May 5 and 12, 1952 are devoted to discussing the results the research and development program in 1950-1951. The minutes of NTS meetings have not been published previously.

    Most schemes for the accreditation (e.g. United Kingdom Accreditation Service) and certification (e.g. BS EN ISO 9002) of laboratories include a requirement to establish and maintain procedures for the management and control of documents generated internally. Such documents include policy statements, procedures, specifications, and some notices and memoranda. Organisations benefit from using agreed and approved information and from knowing that staff are using agreed and approved methods in their operating procedures. Document control systems are likely to become compulsory as accreditation schemes, such as Clinical Pathology Accreditation (CPA) for clinical microbiology laboratories, align with international standards. The Technical Services Division (TSD) in PHLS Headquarters has been developing a control system for various documents that it issues to the PHLS and control of documentation that forms the TSD quality system. The document control system has recently developed into a document management system that provides a mechanism for managing all documents generated or received by the division. TSD's approach is described here to provide laboratories and other organisations with ideas for how they could set up or develop their own document management system to improve accessibility to information.       The charters of Freedom Monitoring System will periodically assess the physical condition of the U.S. Constitution, Declaration of Independence and Bill of Rights. Although protected in helium filled glass cases, the documents are subject to damage from light vibration and humidity. The photometer is a CCD detector used as the electronic film for the camera system's scanning camera which mechanically scans the document line by line and acquires a series of images, each representing a one square inch portion of the document. Perkin-Elmer Corporation's photometer is capable of detecting changes in contrast, shape or other indicators of degradation with 5 to 10 times the sensitivity of the human eye. A Vicom image processing computer receives the data from the photometer stores it and manipulates it, allowing comparison of electronic images over time to detect changes.

      Orbitmpi is a parallelized version of Roscoe White's Orbit code. The code has been parallelized using MPI, which makes it portable to many types of machines. The guidelines used for the parallelization were to increase code performance with minimal changes to the code's original structure. This document gives a general description of how the parallel sections of the code run. It discusses the changes made to the original code and comments on the general procedure for future additions to Orbitmpi, as well as describing the effects of a parallelized random number generator on the code's output. Finally, the scaling results from Hecate and from Puffin are presented.       OMEGA is a CRAY I computer program that controls nine codes used by LLNL Physical Data Group for: 1) updating the libraries of evaluated data maintained by the group (UPDATE); 2) calculating average values of energy deposited in secondary particles and residual nuclei (ENDEP); 3) checking the libraries for internal consistency, especially for energy conservation (GAMCHK); 4) producing listings, indexes and plots of the library data (UTILITY); 5) producing calculational constants such as group averaged cross sections and transfer matrices for diffusion and Sn transport codes (CLYDE); 6) producing and updating standard files of the calculational constants used by LLNL Sn and diffusion transport codes (NDFL); 7) producing calculational constants for Monte Carlo transport codes that use group-averaged cross sections and continuous energy for particles (CTART); 8) producing and updating standard files used by the LLNL Monte Carlo transport codes (TRTL); and 9) producing standard files used by the LANL pointwise Monte Carlo transport code MCNP (MCPOINT). The first four of these functions and codes deal with the libraries of evaluated data and the last five with various aspects of producing calculational constants for use by transport codes. In 1970 a series, called PD memos, of internal and informal memoranda was begun. These were intended to be circulated among the group for comment and then to provide documentation for later reference whenever questions arose about the subject matter of the memos. They have served this purpose and now will be drawn upon as source material for this more comprehensive report that deals with most of the matters covered in those memos.

      In the past decade, stimulated by public concern with issues of mental health, there has been a new spurt of interest in techniques of brief treatment with circumscribed goals. These are applicable to groups as well as to the single patient. There are still differences of opinion about the effectiveness of brief psychotherapy, particularly the lastingness and depth of the results obtained, yet it is often highly beneficial, especially to previously well-functioning individuals who are involved in a situational crisis. Although probably the best results of brief psychotherapy are with disturbances of moderate severity and recent onset, in practice, it is often tried with a wide spectrum of patients. Brief psychotherapy aims at relief of the patient's major current conflicts rather than at change of his personality structure, which generally requires long-term treatment. Brief psychotherapy is of special relevance for the general physician because the patients whom he sees in large numbers are precisely those best suited for this form of treatment. PMID:6075465

      In response to a large public demand for information, the Educational Services Branch of NASA has undertaken a series of publications designed for use by teachers, titled 'Educational Briefs for the Classroom', which has resulted in six to eight issues each year for the last three years. Typical of the topics to which the series is dedicated have been space suits, manned spaceflight mission summaries, solar cells, planetary encounter data, orbits, and rocketry. The planning committee for Educational Briefs is aided in its selection of topics by the many letters received by NASA. Following the Voyager Saturn flybys, NASA received more than 175,000 letters from both children and adults.

      Trainees rotate onto the medical psychiatric inpatient unit at Massachusetts General Hospital every 6 weeks to learn how to conduct brief inpatient psychotherapy from two staff psychologists and one staff psychiatrist. This article focuses on four key therapeutic principles/techniques used when teaching these trainees about brief inpatient psychotherapy.       A large proportion of Americans report binge or heavy drinking. The human and economic costs of alcohol misuse are extensive, with hundreds of thousands of lives lost or disrupted and billions of dollars spent due to impaired productivity, crime, and adverse health consequences. In an effort to reduce costs such as these, scientists and clinicians have developed brief interventions, characterized by their low intensity and short (5-60 minutes) duration, as well as by their intent to provide early intervention before drinkers develop alcohol abuse or dependence. The purpose of this review, therefore, is to analyze research studies related to brief intervention and critically analyze and critique their findings. In addition, both prospective randomized controlled trials and meta-analyses will be used to discuss the implications for clinical practice and make recommendations for future research.       A major step in transferring technology is to disseminate information about new developments to the appropriate sector(s). A useful vehicle for transferring technology from the government sector to industry has been demonstrated with the use of periodical and journal announcements to highlight technological achievements which may meet the needs of industries other than the one who developed the innovation. To meet this end, NASA has very successfully pursued the goal of identifying technical innovations through the national circulation publication; NASA Tech Briefs. At one time the Technology Utilization Offices of the various centers coordinated the selection of appropriate technologies through a common channel. In recent years, each NASA field center has undertaken the task of evaluating submittals for Tech Brief publication independently of the others. The University of Alabama in Huntsville was selected to assist MSFC in evaluating technology developed under the various programs managed by the NASA center for publication in the NASA Tech Briefs journal. The primary motivation for the NASA Tech Briefs publication is to bring to the attention of industry the various NASA technologies which, in general, have been developed for a specific aerospace requirement, but has application in other areas. Since there are a number of applications outside of NASA that can benefit from innovative concepts developed within the MSPC programs, the ability to transfer technology to other sectors is very high. In most cases, the innovator(s) are not always knowledgeable about other industries which might potentially benefit from their innovation. The evaluation process can therefore contribute to the list of potential users through a knowledgeable evaluator.

      The Strategic Forces Briefing'' is our attempt, accomplished over the past several months, to outline and highlight the more significant strategic force issues that must be addressed in the near future. Some issues are recurrent: the need for an effective modernized Triad and a constant concern for force survivability. Some issues derive from arms control: the Strategic Arms Reduction Talks (SALT) are sufficiently advanced to set broad numerical limits on forces, but not so constraining as to preclude choices among weapon systems and deployment modes. Finally, a new administration faced with serious budgetary problems must strive for the most effective strategic forces limited dollars can buy and support. A review of strategic forces logically begins with consideration of the missions the forces are charged with. We begin the briefing with a short review of targeting policy and implementation within the constraints of available unclassified information. We then review each element of the Triad with sections on SLBMs, ICBMs, and Air-Breathing (bomber and cruise missile) systems. A short section at the end deals with the potential impact of strategic defense on offensive force planning. We consider ABM, ASAT, and air defense; but we do not attempt to address the technical issues of strategic defense per se. The final section gives a brief overview of the tritium supply problem. We conclude with a summary of recommendations that emerge from our review. The results of calculation on the effectiveness of various weapon systems as a function of cost that are presented in the briefing are by Paul Chrzanowski.

      A software application has been developed to aid law enforcement and government intelligence gathering organizations in the translation and analysis of foreign language documents with potential intelligence content. The Automated Document Analysis System (ADAS) provides the capability to search (data or text mine) documents in English and the most commonly encountered foreign languages, including Arabic. Hardcopy documents are scanned by a high-speed scanner and are optical character recognized (OCR). Documents obtained in an electronic format bypass the OCR and are copied directly to a working directory. For translation and analysis, the script and the language of the documents are first determined. If the document is not in English, the document is machine translated to English. The documents are searched for keywords and key features in either the native language or translated English. The user can quickly review the document to determine if it has any intelligence content and whether detailed, verbatim human translation is required. The documents and document content are cataloged for potential future analysis. The system allows non-linguists to evaluate foreign language documents and allows for the quick analysis of a large quantity of documents. All document processing can be performed manually or automatically on a single document or a batch of documents.

      Taxometric procedures provide an empirical means of determining which psychiatric disorders are typologically distinct from normal behavioral functioning. Although most disorders reflect extremes along continuously distributed behavioral traits, identifying those that are discrete has important implications for accurate diagnosis, effective treatment, early identification of risk, and improved understanding of etiology. This article provides (a) brief descriptions of the conceptual bases of several taxometric procedures, (b) example analyses using simulated data, and (c) strategies for avoiding common pitfalls that are often observed in taxometrics research. To date, most taxometrics studies have appeared in the adult psychopathology literature. During the question and answer period, the issue of the use of plutonium on the mission is broached, which engenders a review of the testing methods used to ensure the safety of the capsules containing the hazardous substance.       Poor quality documents are obtained in various situations such as historical document collections, legal archives, security investigations, and documents found in clandestine locations. Such documents are often scanned for automated analysis, further processing, and archiving. Due to the nature of such documents, degraded document images are often hard to read, have low contrast, and are corrupted by various artifacts. We describe a novel approach for the enhancement of such documents based on probabilistic models which increases the contrast, and thus, readability of such documents under various degradations. The enhancement produced by the proposed approach can be viewed under different viewing conditions if desired. The proposed approach was evaluated qualitatively and compared to standard enhancement techniques on a subset of historical documents obtained from the Yad Vashem Holocaust museum. In addition, quantitative performance was evaluated based on synthetically generated data corrupted under various degradation models. Preliminary results demonstrate the effectiveness of the proposed approach.

The model achieves a supply and demand balance in the end-use demand regions, defined as the nine Census Divisions, by solving for the prices of each energy type such that the quantities producers are willing to supply equal the quantities consumers wish to consume. The system reflects market economics, industry structure, and energy policies and regulations that influence market behavior. The NEMS Integrating Module is the central integrating component of a complex modeling system. The NASA Software Documentation Standard can be applied to the documentation of all NASA software. The Standard is limited to documentation format and content requirements. It does not mandate specific management, engineering, or assurance standards or techniques.       Design: Interpretive analysis of published research. Sample: 173 papers indexed in Medline between 1995 and 2004 that cited tobacco industry documents. Analysis: Information about year published, journal and author, and a set of codes relating to methods reporting, were managed in N*Vivo. This coding formed the basis of an interpretation of tobacco document research reporting. Results: Two types of papers were identified. The first used tobacco documents as the primary data source (A-papers). The second was dedicated to another purpose but cited a small number of documents (B-papers). In B-papers documents were used either to provide a specific example or to support an expansive contention. A-papers contained information about purpose, sources, searching, analysis, and limitations that differed by author and journal and over time. A-papers had no clear methodological context, but used words from three major traditions—interpretive research, positivist research, and history—to describe analysis. Interpretation: A descriptive mainstream form of tobacco document reporting is proposed, initially typical but decreasing, and a continuum of positioning of the researcher, from conduit to constructor. Reporting practices, particularly from experienced researchers, appeared to evolve towards researcher as constructor, with later papers showing more complex purposes, diverse sources, and detail of searching and analysis. Tobacco document research could learn from existing research traditions: a model for planning and evaluating tobacco document research is presented. PMID:16319359

      A large number of patients with psychosocial or psychiatric disorders present to family physicians, and the family physician needs a model of psychotherapy with which to cope with their problems. A model of brief psychotherapy is presented which is time limited, goal directed and easy to learn. It consists of four facets drawn from established areas of psychotherapy: characteristics of the therapist; characteristics of the patient; Eriksonian developmental stages; and the process of therapy as described by Carkhuff.       Patient documentation is time consuming and can detract from care. The authors report a novel computer programme that manipulates routinely collected information to quantify nursing workload, along with the reason for admission, functional status, estimates of in-hospital mortality and life expectancy. The programme stores information in a database, and produces a print-out in a situation/background/assessment/recommendation (SBAR) format. The average time taken to enter 629 patient encounters was 6.6 minutes. Pain was the most common presentation for low workload patients, while high workload patients often presented with altered mental status and reduced mobility. There was only a modest correlation between the risk of death and nursing workload. The programme measures nursing workload without further paperwork, and improves routine documentation with a legible brief report that is automatically generated. This report can be shared and provides data that is immediately available for day-to-day care, audit, quality control and service planning. PMID:23249801

      To analyze if an iPad-based patient briefing can serve as a digital alternative to conventional documentations prior to radiological examinations. One hundred one patients referred for routine MRI were randomized into two groups, who underwent iPad-based and classic written briefing in opposite order. For each briefing completion time, completeness and correctness were noted. Patient's knowledge about the content of either briefing modality was subsequently tested. The influence of patient-related factors on the performance of the electronic briefing (EB) was analyzed. Finally, the patient's subjective impression of the EB was assessed. The mean durations were 4.4 ± 2.2 min for EB and 1.7 ± 1.3 min for the classic briefing (p < 0.01). All iPad briefings were returned entirely filled out, whereas 11 % of the classic forms were returned with missing data. No significant differences in memorization of the briefing's information were objectified. There was a positive correlation between the duration of EB and age (r = 0.53; p < 0.01), whereas a negative correlation was found between computer skills and patient's age (r = -0.55; p < 0.01) or duration of EB (r = -0.62; p < 0.01). More than half of the study patients would prefer EB in the future; another 29 % had no preference at all. Patient briefing on iPads transfers the information for the patients equally well compared to the classic written approach. Although iPad briefing took patients longer to perform, the majority would prefer it to written consent briefings in the future. Nevertheless, measures have to be undertaken to improve the overall acceptance and performance. PMID:24687643

We also included exportability such as attaching additional documents that can be downloaded from the web application. The finished application will ease the process of managing Content Documents while streamlining the procedure. Ruby on Rails is a very powerful programming language and I am grateful to have the opportunity to build this application.

      A briefing on isotope systems technology and applications was held at the Pentagon on 9 May 1990, cosponsored by the Department of Defense and the Department of Energy. The objective was to provide isotope systems information to interested individuals and offices from the DOD and the Services, and then to discuss potential uses of this technology within the defense community. Presentations were given on the useful characteristics of radioactive decay, isotope sources, properties of specific isotopes, and details of isotope systems that have been deployed for terrestrial and space applications. Application areas covered included: electrical, thermal and mechanical energy conversion systems; radiation applications; and radioluminescent lights. The telescope is scheduled to be launched during the STS-93 flight. The participants in the briefing are: Don Savage, of NASA Headquarters; Ed Weiler, Associate Administrator for Space Sciences; Alan Bunner, Chandra Project Scientist and Michael Turner, an astrophysicist at the University of Chicago.       This article presents a study of how humans perceive and judge the relevance of documents. Humans are adept at making reasonably robust and quick decisions about what information is relevant to them, despite the ever increasing complexity and volume of their surrounding information environment. The literature on document relevance has identified various dimensions of relevance (e.g., topicality, novelty, etc.), however little is understood about how these dimensions may interact. We performed a crowdsourced study of how human subjects judge two relevance dimensions in relation to document snippets retrieved from an internet search engine. The order of the judgment was controlled. For those judgments exhibiting an order effect, a q–test was performed to determine whether the order effects can be explained by a quantum decision model based on incompatible decision perspectives. Some evidence of incompatibility was found which suggests incompatible decision perspectives is appropriate for explaining interacting dimensions of relevance in such instances. PMID:25071622

      The main technical issues of the Computer Integrated Documentation (CID) project are presented. The problem of automation of documents management and maintenance is analyzed both from an artificial intelligence viewpoint and from a human factors viewpoint. Possible technologies for CID are reviewed: conventional approaches to indexing and information retrieval; hypertext; and knowledge based systems. A particular effort was made to provide an appropriate representation for contextual knowledge. This representation is used to generate context on hypertext links. Thus, indexing in CID is context sensitive. The implementation of the current version of CID is described. It includes a hypertext data base, a knowledge based management and maintenance system, and a user interface.       We report an investigation into strategies, algorithms, and software tools for document image content extraction and inventory, that is, the location and measurement of regions containing handwriting, machine-printed text, photographs, blank space, etc. We have developed automatically trainable methods, adaptable to many kinds of documents represented as bilevel, greylevel, or color images, that offer a wide range of useful tradeoffs of speed versus accuracy using methods for exact and approximate k-Nearest Neighbor classification. We have adopted a policy of classifying each pixel (rather than regions) by content type: we discuss the motivation and engineering implications of this choice. We describe experiments on a wide variety of document-image and content types, and discuss performance in detail in terms of classification speed, per-pixel classification accuracy, per-page inventory accuracy, and subjective quality of page segmentation. These show that even modest per-pixel classification accuracies (of, e.g., 60-70%) support usefully high recall and precision rates (of, e.g., 80-90%) for retrieval queries of document collections seeking pages that contain a given minimum fraction of a certain type of content.

    4. Extremely secure identification documents

      The technology developed in this project uses biometric information printed on the document and public key cryptography to ensure that an adversary cannot issue identification documents to unauthorized individuals or alter existing documents to allow their use by unauthorized individuals. This process can be used to produce many types of identification documents with much higher security than any currently in use. The system is demonstrated using a security badge as an example. This project focused on the technologies requiring development in order to make the approach viable with existing badge printing and laminating technologies. By far the most difficult was the image processing required to verify that the picture on the badge had not been altered. Another area that required considerable work was the high density printed data storage required to get sufficient data on the badge for verification of the picture. The image processing process was successfully tested, and recommendations are included to refine the badge system to ensure high reliability. A two dimensional data array suitable for printing the required data on the badge was proposed, but testing of the readability of the array had to be abandoned due to reallocation of the budgeted funds by the LDRD office.

DOCUTECH provides the capability to print on demand electronically stored documents. Use of the electronic publishing system in several applications has generated productivity gains ranging from 33 to 88 percent at geographically dispersed NASA centers.

    9. Kawasaki disease: A brief history.

      Tomisaku Kawasaki published the first English-language report of 50 patients with Kawasaki disease (KD) in 1974. Since that time, KD has become the leading cause of acquired heart disease among children in North America and Japan. Although an infectious agent is suspected, the cause remains unknown. However, significant progress has been made toward understanding the natural history of the disease and therapeutic interventions have been developed that halt the immune-mediated destruction of the arterial wall. We present a brief history of KD, review progress in research on the disease, and suggest avenues for future study. Kawasaki saw his first case of KD in January 1961 and published his first report in Japanese in 1967. Whether cases existed in Japan before that time is currently under study. The most significant controversy in the 1960s in Japan was whether the rash and fever sign/symptom complex described by Kawasaki was connected to subsequent cardiac complications in a number of cases. Pathologist Noboru Tanaka and pediatrician Takajiro Yamamoto disputed the early assertion of Kawasaki that KD was a self-limited illness with no sequelae. This controversy was resolved in 1970 when the first Japanese nationwide survey of KD documented 10 autopsy cases of sudden cardiac death after KD. By the time of the first English-language publication by Kawasaki in 1974, the link between KD and coronary artery vasculitis was well-established. KD was independently recognized as a new and distinct condition in the early 1970s by pediatricians Marian Melish and Raquel Hicks at the University of Hawaii. In 1973, at the same Hawaiian hospital, pathologist Eunice Larson, in consultation with Benjamin Landing at Los Angeles Children's Hospital, retrospectively diagnosed a 1971 autopsy case as KD. The similarity between KD and infantile periarteritis nodosa (IPN) was apparent to these pathologists, as it had been to Tanaka earlier. What remains unknown is the reason for the

      The Uranium Mill Tailings Radiation Control Act (UMTRCA) of 1978, Public Law 95-604 (PL95-604), grants the Secretary of Energy the authority and responsibility to perform such actions as are necessary to minimize radiation health hazards and other environmental hazards caused by inactive uranium mill sites. This Technical Approach Document (TAD) describes the general technical approaches and design criteria adopted by the US Department of Energy (DOE) in order to implement remedial action plans (RAPS) and final designs that comply with EPA standards. It does not address the technical approaches necessary for aquifer restoration at processing sites; a guidance document, currently in preparation, will describe aquifer restoration concerns and technical protocols. This document is a second revision to the original document issued in May 1986; the revision has been made in response to changes to the groundwater standards of 40 CFR 192, Subparts A--C, proposed by EPA as draft standards. New sections were added to define the design approaches and designs necessary to comply with the groundwater standards. These new sections are in addition to changes made throughout the document to reflect current procedures, especially in cover design, water resources protection, and alternate site selection; only minor revisions were made to some of the sections. Sections 3.0 is a new section defining the approach taken in the design of disposal cells; Section 4.0 has been revised to include design of vegetated covers; Section 8.0 discusses design approaches necessary for compliance with the groundwater standards; and Section 9.0 is a new section dealing with nonradiological hazardous constituents. 203 refs., 18 figs., 26 tabs.
