  1. Alternative fuel vehicles for the state fleets: Results of the 5-year planning process

    This report documents the first attempt by the Department of Energy (DOE) to work with states to prepare five-year Alternative Fuel Vehicle (AFV) acquisition plans to identify alternative fuels and vehicles that they are planning on or would like to acquire. The DOE Regional Support Offices (RSOs) met with representatives from the states in their regions and assisted in the preparation of the plans. These plans will be used in conjunction with previously gathered Federal five-year plans to encourage Original Equipment Manufacturers (OEMs) to expand the variety of AFVs produced, reduce the incremental cost of AFVs, and to encourage fuel suppliers to expand the alternative fuel infrastructure and alternative fuel availability. By identifying the needs and requirements of state fleets, DOE can begin to describe the specific nature of the future state fleets, and establish a defined market for OEMs and fuel suppliers. DOE initiated the development and collection of the state five-year plans before the signing of the Energy Policy Act, to raise the awareness of states that they will be required by law to acquire AFVs. As a result, several states that had no AFV acquisition plan when queried have developed or are in the process of developing plans. The DOE and its RSOs are still working with the states to develop and refine acquisition plans, and this report should be treated as documentation of work in progress.

    This report describes five-year plans for acquisition of alternative fuel vehicles (AFVs) by the Federal agencies. These plans will be used to encourage Original Equipment Manufacturers (OEMs) to expand the variety of AFVs produced, reduce the incremental cost of AFVs, and to encourage fuel suppliers to expand the alternative fuel infrastructure and alternative fuel availability. This effort supplements and extends the demonstration and testing of AFVs established by the Department of Energy under the alternative Motor Fuels Act of 1988.

    A broad range of scientific goals and a similarly diverse set of consumers drive the informatics requirements and computing needs of the JGI. The scope of work in this area encompasses not only the informatics and analysis pipelines in support of the PGF sequence production, but also the integration of data from a variety of sources and sophisticated large scale analyses led by investigators within JGI and driven by the user science community. In laying out a forward looking strategy, the full range of these activities need to be examined together to build a comprehensive program that will serve as a catalyst for the DOE research community. The science landscape envisioned in the overall strategic plan calls for significantly increasing the throughput of microbial genomes sequenced to cover their phylogenetic space and building a set of finished reference plant genomes to enable DOE relevant science. Additionally, the established impact of microbial communities on global energy cycles and their potential in remediation endeavors, warrant building upon JGI's established expertise in metagenomic analysis. Not only is each of these program areas relevant and exciting in their own right, but they also can and should be undertaken in a way that allows synthesis across domains (e.g. utilize knowledge from sequence of plants and the soil from which they are grown). Both dramatic increases in the scale of genomic data collection and the synergistic potential of integrating data across domains will demand new strategies in the informatics pipeline within the JGI and in the facility's approach to computational analysis and user access to the data in aggregated form. In addition to a robust and scalable informatics infrastructure, fulfilling the strategic science goals of the JGI will require ongoing investment in usability of the data, to ensure that the data collected will be used to maximal effect.     Maine EPSCoR has developed a five year plan to further improve Maine`s research and education capacity in the field of Energy. The initiatives of this Energy Education and Research Plan are integrated with other major science policy initiatives in the state, specifically the state`s Science and Technology Strategic Plan (1992), the NSF Statewide Systemic Initiative (1992), and the Report of the Maine Commission on Comprehensive Energy Planning. The plan was developed with the support of US Department of Energy and State of Maine funds. The planning process was led by the Maine DOE EPSCoR planning committee of Maine EPSCoR. Researchers, educators, and business people assisted the committee in the development of the plan. This plan draws from priorities established by focus groups, the strengths and weaknesses revealed by the resource assessment, and the suggestions offered in the solicited research and education briefs. The plan outlines strategies for the improvement of energy education, communication networks, support of individual research, and the formation of collaborative research groups in targeted areas. Five energy-related areas have been targeted for possible development of collaborative research groups: Energy Technology Research, Energy and the Environment, the Gulf of Maine and Its Watershed, the Human Genome, and Renewable Energy. The targeted areas are not boundaries limiting the extent of collaborations to be pursued but represent research themes through which the state`s resources can be combined and improved.

    AYUSH is an acronym for Ayurveda, Yoga and Naturopathy, Unani, Siddha, and Homeopathy. These are the six indigenous systems of medicine practiced in India. A department called Department of Indian System of medicine was created in March 1995 and renamed to AYUSH in November 2003 with a focus to provide increased attention for the development of these systems. Very recently, in 2014, a separate ministry was created under the union Government of India, which is headed by a minister of state. Planning regarding these systems of medicine was a part of 5-year planning process since 1951. Since then many developments have happened in this sector albeit the system was struggling with a great degree of uncertainty at the time of 1(st)5-year plan. A progressive path of development could be observed since the first to the 12(th)5-year plan. It was up to the 7(th)plan the growth was little sluggish and from 8(th)plan onward the growth took its pace and several innovative development processes could be observed thereafter. The system is gradually progressing ahead with a vision to be a globally accepted system, as envisaged in 11(th)5-year plan. Currently, AYUSH system is a part of mainstream health system implemented under National Rural Health Mission (NRHM). NRHM came into play in 2005 but implemented at ground level in 2006 and introduced the scheme of "Mainstreaming of AYUSH and revitalization of local health traditions" to strengthen public health services. This scheme is currently in operation in its second phase, since 1(st)April 2012, with the 12(th)5-year plan. The scheme was primarily brought in to operation with three important objectives; choice of treatment system to the patients, strengthen facility functionally and strengthen the implementation of national health programmes, however, in some places it seems to be a forced medical pluralism owing to a top-down approach by the union government without considerable involvement of the concerned community.     The US Department of Energy's (DOE) Resource Assessment Program produces scientific descriptions and assessments of the nation's renewable energy resources, such as solar energy. Information about the resources --- for example, how solar energy varies with location and climate --- is required to develop energy conversion technologies, design and site systems, and forecast the systems' performance. With information about resource availability and renewable energy system performance, DOE can assess the potential for renewable energy to contribute to the nation's energy supply as part of the long-term national energy strategy. This 5-year plan for fiscal years (FY) 1991 through 1995 gives the strategy to produce solar radiation resource characterizations and assessments under the DOE project at SERI. It is consistent with the mini-multiyear plan for resource assessment prepared by DOE in 1989 and incorporates the comments received at a project overview held in April 1990 at DOE Headquarters. 7 figs.

    Access to the right to the highest attainable level of health is a constitutional right that obliges governments and other players to take step to increase all individuals' chances of obtaining good health. At the least, health and education are two crucial requirements for this as well. Iran's vision 2025 is going to lead the country to a developed state with the highest rank of economic, scientific and technological status in the region. Enjoying health, welfare, food security, social security, equal opportunities, etc, are also considered as part of characteristics of Iranian society in 2025. Although health system of Iran has many achievements in providing health services specially for the poor following the Islamic Revolution of 1979, but the evidences gathered to develop the 5(th) 5-years economical, social and cultural plan (5(th)5YDP:2011-2015), listed a variety of main challenges in stewardship, financing, resources generation and service provision functions of the existing health system. Thus, to overcome the main challenges, about 11% of general policies of 5(th)5YDP are directly address health related issues with emphasizing on healthy human and comprehensive health approach with considering: Integration of policy making, planning, evaluation, supervision and public financing; Developing both quantity and quality of health insurance system and reducing out-of-pocket expenditures for health services to 30% by the end of the 5th plan. The strategies of 5(th)5YDP adopted by the parliament as an Act will change the health system fundamentally through tuning the main drivers; so, its implementation needs brave leaders, capable managers, motivated technical staff and social mobilization.   2. Soft tissue stability after segmental distraction of the anterior mandibular alveolar process: a 5.5 year follow-up.

    Soft tissue changes were analysed retrospectively in 17 patients following distraction osteogenesis (DO) of the mandibular anterior alveolar process. Lateral cephalograms were traced by hand, digitized, superimposed, and evaluated at T1 (17.0 days), after DO at T2 (mean 6.5 days), at T3 (mean 24.4 days), at T4 (mean 2.0 years), and at T5 (mean 5.5 years). Statistical analysis was carried out using Kolmogorov-Smirnov test, paired t-test, Pearson's correlation coefficient, and linear backward regression analysis. 5.5 years postoperatively, the net effect for the soft tissue at point B' was 88% of the advancement at point B while the lower lip (labrale inferior) followed the advancement of incision inferior to 24%. Increased preoperative age was correlated (p<0.05) with more horizontal backward movement (T5-T3) for labrale inferior and pogonion'. Higher NL/ML' angles were significantly correlated (p<0.05) to smaller horizontal soft tissue change at labrale inferior (T5-T3). The amount of advancement at point B was significantly correlated with an upward movement (T5-T3) of labrale inferior (p<0.01) and stomion inferior (p<0.05). It can be concluded that further change in soft tissues occurred between 2.0 and 5.5 years postoperatively. The physiological process of ageing and loss of soft tissue elasticity should be considered as possible reasons. PMID:23182400

The presentation provides a description of: where the requirements originate, to whom they are submitted, how they are integrated into a requirements plan, and how that integrated plan is formulated and approved. It is hoped that from completing the review of this presentation, one will get an understanding of the planning process that formulates payload requirements into an integrated plan used for specifying research activities to take place on the ISS.

  5. Cassini science planning process

    The mission design for Cassini-Huygens calls for a four-year orbital survey of the Saturnian system and the descent into the Titan atmosphere and eventual soft-landing of the Huygens probe. The Cassini orbiter tour consists of 76 orbits around Saturn with 44 close Titan flybys and 8 targeted icy satellite flybys. The Cassini orbiter spacecraft carries twelve scientific instruments that will perform a wide range of observations on a multitude of designated targets. The science opportunities, frequency of encounters, the length of the Tour, and the use of distributed operations pose significant challenges for developing the science plan for the orbiter mission. The Cassini Science Planning Process is the process used to develop and integrate the science and engineering plan that incorporates an acceptable level of science required to meet the primary mission objectives far the orbiter. The bulk of the integrated science and engineering plan will be developed prior to Saturn Orbit Insertion (Sol). The Science Planning Process consists of three elements: 1) the creation of the Tour Atlas, which identifies the science opportunities in the tour, 2) the development of the Science Operations Plan (SOP), which is the conflict-free timeline of all science observations and engineering activities, a constraint-checked spacecraft pointing profile, and data volume allocations to the science instruments, and 3) an Aftermarket and SOP Update process, which is used to update the SOP while in tour with the latest information on spacecraft performance, science opportunities, and ephemerides. This paper will discuss the various elements of the Science Planning Process used on the Cassini Mission to integrate, implement, and adapt the science and engineering activity plans for Tour.

    The software program is called Computer Managed Process Planning, (CMPP). This system employs computer technology to speed and simplify process planning of cylindrical parts. It is aimed at the machining of expensive materials requiring tight tolerances and complex manufacturing processes. Thus far, the program has resulted in a 75 percent reduction in manpower and a 70 percent increase in productivity. The software performs four tasks: (1) it manipulates the manufacturing database, accepting input information from a central CAD system or from an interactive display; (2) it finds the dimensioning reference surfaces, clamping surfaces, and locating surfaces for each machining operation; (3) it analyzes dimensions, tolerances, and stock removals in all cuts for each operation to ensure that bluprint specifications can be achieved; and (4) it prints a summary of operations or a routing sheet.

    Five years of Japanese national project (FY2003-FY2007) was ended last spring with remarkable success. The national project was originally aimed for development of coated conductors which have high superconductivity performance and long length enough to fabricate high temperature superconducting (HTS) electric power devices. Preliminary research and development of HTS electric power devices were carried out as well. A series of R&D results will be summarized and reviewed in this paper. The new 5 years Japanese national project has started last June (FY2008-FY2012) to develop HTS electric power applications including SMES, cables and transformers and to develop/produce coated conductors satisfying the requirements from the respective power devices. Collaborative R&D efforts by national laboratories, universities and private companies have been started with the supports of NEDO and METI. The accomplishment of the previous project will be summarized and the plans and goals of the new project will be presented in this paper.

    17 patients (14 female; 3 male) were analysed retrospectively for skeletal and dental relapse before distraction osteogenesis (DO) of the mandibular anterior alveolar process at T1 (17.0 days), after DO at T2 (mean 6.5 days), at T3 (mean 24.4 days), at T4 (mean 2.0 years), and at T5 (mean 5.5 years). Lateral cephalograms were traced by hand, digitized, superimposed, and evaluated. Skeletal correction (T5-T1) was mainly achieved through the distraction of the anterior alveolar segment in a rotational manner where the incisors were more proclined. The horizontal backward relapse (T5-T3) measured -0.3mm or 8.3% at point B (non-significant) and -1.8mm or 29.0% at incision inferior (p<0.01). Age, gender, amount and type (rotational vs. translational) of advancement were not correlated with the amount of relapse. High angle patients (NL/ML'; p<0.01) showed significant smaller relapse rates at point B. Overcorrection of the overjet achieved by the distraction could be a reason for dental relapse. Considering the amount of long-term skeletal relapse the DO could be an alternative to bilateral sagittal split osteotomy for mandibular advancement in selected cases. PMID:23153785

    Due to their geographic location and traditional diet, rich in seafood and marine mammals, the Inuit living in Arctic Quebec are exposed to high amounts of pollutants, including organochlorine pesticides (OCPs). While the adverse developmental effects of these pesticides on child cognitive functions are well known, the effects of developmental exposure to OP on sensory processes have not been investigated. The aim of this longitudinal study was to assess the effects of prenatal and childhood exposure to 1,1,1-trichloro-2,2-bis(p-chlorophenyl)ethane (p,p'-DDT) and its major metabolite 1,1,-dichloro-2,2-bis(p-chlorophenyl)ethylene (p,p'-DDE), on visual processing in Inuit children in Nunavik (Arctic Québec). p,p'-DDT and p,p'-DDE concentrations were determined from umbilical cord and 5- and 11-year plasma samples. Visual evoked potentials (VEPs) were successfully recorded in 150 children at 4 contrast levels (95%, 30%, 12%, and 4%). Hierarchical multiple regressions were conducted to determine the association between p,p'-DDT, or p,p'-DDE, exposure and VEPs while controlling for the effects of various confounders, including fish nutrients and other contaminants. p,p'-DDE measured in umbilical cord plasma was significantly related to the amplitude of the N150 response at the lowest contrast (4%). In addition, 5-year p,p'-DDE plasma concentration was significantly associated with decreased N75 amplitude. These findings indicate that p,p'-DDE exposure, both pre- and postnatally, during early childhood is associated with visual processing impairment later in life. PMID:24812027

    Generative process planning describes methods process engineers use to modify manufacturing/process plans after designs are complete. A completed design may be the result from the introduction of a new product based on an old design, an assembly upgrade, or modified product designs used for a family of similar products. An engineer designs an assembly and then creates plans capturing manufacturing processes, including assembly sequences, component joining methods, part costs, labor costs, etc. When new products originate as a result of an upgrade, component geometry may change, and/or additional components and subassemblies may be added to or are omitted from the original design. As a result process engineers are forced to create new plans. This is further complicated by the fact that the process engineer is forced to manually generate these plans for each product upgrade. To generate new assembly plans for product upgrades, engineers must manually re-specify the manufacturing plan selection criteria and re-run the planners. To remedy this problem, special-purpose assembly planning algorithms have been developed to automatically recognize design modifications and automatically apply previously defined manufacturing plan selection criteria and constraints.

    Process planning has been recognized as an interface between computer-aided design and computer-aided manufacturing. Since the late 1960s, computer techniques have been used to automate process planning activities. AI-based techniques are designed for capturing, representing, organizing, and utilizing knowledge by computers, and are extremely useful for automated process planning. To date, most of the AI-based approaches used in automated process planning are some variations of knowledge-based expert systems. Due to their knowledge acquisition bottleneck, expert systems are not sufficient in solving process planning problems. Fortunately, AI has developed other techniques that are useful for knowledge acquisition, e.g., neural networks. Neural networks have several advantages over expert systems that are desired in today's manufacturing practice. However, very few neural network applications in process planning have been reported. We present this paper in order to stimulate the research on using neural networks for process planning. This paper also identifies the problems with neural networks and suggests some possible solutions, which will provide some guidelines for research and implementation.

    Periodic microbiological monitoring of endoscopes is a recommendation of the Gastroenterological Society of Australia (GENSA). The aim of monitoring has been to provide quality assurance of the cleaning and disinfection of endoscopes; however, there is controversy regarding its frequency. This lack of consensus stimulated a review of the experience within our health service. At Southern Health, routine microbiological sampling has involved 4-weekly monitoring of bronchoscopes, duodenoscopes and automated flexible endoscope reprocessors (AFER), and 3-monthly monitoring of all other gastrointestinal endoscopes. Records of testing were reviewed from 1 January 2002 until 31 December 2006. A literature review was conducted, cost analysis performed and positive cultures investigated. There were 2374 screening tests performed during the 5-year period, including 287 AFER, 631 bronchoscopes for mycobacteria and 1456 endoscope bacterial screens. There were no positive results of the AFER or bronchoscopes for mycobacteria. Of the 1456 endoscopic bacterial samples, six were positive; however, retesting resulted in no growth. The overall cost of tests performed and cost in time for nursing staff to collect the samples was estimated at $AUD 100,400. Periodic monitoring of endoscopes is both time-consuming and costly. Our review demonstrates that AFER (Soluscope) perform well in cleaning endoscopes. Based on our 5-year experience, assurance of quality for endoscopic use could be achieved through process control as opposed to product control. Maintenance of endoscopes and AFER should be in accordance with the manufacturer's instructions and microbiological testing performed on commissioning, annually and following repair. Initial prompt manual leak testing and manual cleaning followed by mechanical leak testing, cleaning and disinfection should be the minimum standard in reprocessing of endoscopes. PMID:18086113

    OBJECTIVE: This single-institution experience retrospectively reviews the outcomes in a group of patients treated 5 or more years ago by pancreaticoduodenectomy for periampullary adenocarcinoma. SUMMARY BACKGROUND DATA: Controversy exists regarding the benefit of resection for periampullary adenocarcinoma, particularly for pancreatic tumors. Many series report only Kaplan-Meier actuarial 5-year survival rates. There are believed to be discrepancies between the actuarial 5-year survival data and the actual 5-year survival rates. METHODS: From April 1970 through May 1992, 242 patients underwent pancreaticoduodenal resection for periampullary adenocarcinoma at The Johns Hopkins Hospital. Follow-up was complete through May 1997. All pathology specimens were reviewed and categorized. Actual 5-year survival rates were calculated. The demographic, intraoperative, pathologic, and postoperative features of patients surviving > or =5 years were compared with those of patients who survived <5 years. RESULTS: Of the 242 patients with resected periampullary adenocarcinoma, 149 (62%) were pancreatic primaries, 46 (19%) arose in the ampulla, 30 (12%) were distal bile duct cancers, and 17 (7%) were duodenal cancers. There was a 5.3% operative mortality rate during the 22 years of the review, with a 2% operative mortality rate in the last 100 patients. There were 58 5-year survivors, 28 7-year survivors, and 7 10-year survivors. The tumor-specific 5-year actual survival rates were pancreatic 15%, ampullary 39%, distal bile duct 27%, and duodenal 59%. When compared with patients who did not survive 5 years, the 5-year survivors had a significantly higher percentage of well-differentiated tumors (14% vs. 4%; p = 0.02) and higher incidences of negative resection margins (98% vs. 73%, p < 0.0001) and negative nodal status (62% vs. 31%, p < 0.0001). The tumor-specific 10-year actuarial survival rates were pancreatic 5%, ampullary 25%, distal bile duct 21%, and duodenal 59%. CONCLUSIONS

    There is a lack of information flow from job shop environment to the CAPP (computer aided process planning) system, which prevents a CAPP system from being more practical. THCAPP-G (CAPP for garment, developed by Tsinghua University in 1994) is a two-stage, nonlinear, closed-loop and dynamic process planning system. It generates process plans taking advantage of the flexibility of the manufacturing process and job shop environment, according to the dynamic status of the job shop environment. Techniques of ES (expert system), CPA (critical path analysis), and heuristic method were utilized comprehensively. An information flow between job shop environment and CAPP system was realized in this system. The developed ideas and system structure are discussed mainly in this paper.

    Insights into teachers' planning of mathematics reported here were gathered as part of a broader project examining aspects of the implementation of the Australian curriculum in mathematics (and English). In particular, the responses of primary and secondary teachers to a survey of various aspects of decisions that inform their use of curriculum documents and assessment processes to plan their teaching are discussed. Teachers appear to have a clear idea of the overall topic as the focus of their planning, but they are less clear when asked to articulate the important ideas in that topic. While there is considerable diversity in the processes that teachers use for planning and in the ways that assessment information informs that planning, a consistent theme was that teachers make active decisions at all stages in the planning process. Teachers use a variety of assessment data in various ways, but these are not typically data extracted from external assessments. This research has important implications for those responsible for supporting teachers in the transition to the Australian Curriculum: Mathematics.

    When a product reaches its end of life, there are several options available for processing it including reuse, remanufacturing, recycling, and disposing (the least desirable option). In almost all cases, a certain level of disassembly may be necessary. Thus, finding an optimal (or near optimal) disassembly sequence is crucial to increasing the efficiency of the process. Disassembly operations are labor intensive, can be costly, have unique characteristics and cannot be considered as reverse of assembly operations. Since the complexity of determining the best disassembly sequence increases with the increase in the number of parts of the product, it is extremely crucial that an efficient methodology for disassembly process planning be developed. In this paper, we present a genetic algorithm for disassembly process planning. A case example is considered to demonstrate the functionality of the algorithm.

An explanation of the Part A Permit Application, Form 3 submitted with this document is provided at the beginning of the Part A Section. The closure plan consists of nine chapters and six appendices. The 300 Area Process Trenches received dangerous waste discharges from research and development laboratories in the 300 Area and from fuels fabrication processes. This waste consisted of state-only toxic (WT02), corrosive (D002), chromium (D007), spent halogenated solvents (F001, F002, and F003), and spent nonhalogented solvent (F005).     The role of wildfire in accelerating erosion rates for a certain period after fire has been well documented. Much less information is available on the erosion rates and processes after planned fires that typically burn at much lower intensity. Observational evidence, and some studies in southern and southeastern Australia suggest that erosion after planned fire can be significant if rainfall intensities exceed critical intensities and durations. Understanding erosion processes and rates under these event conditions is of critical importance for planning of burn locations away from critical human assets such as water supplies and infrastructure. We conducted concentrated flow experiments with the purpose to understand what critical conditions are required for significant erosion to occur on planned burn hillslopes. Concentrated flow runon was applied on pre-wetted, unbounded plots of 10 m at rates of 0.5, 1, 1.5 and 2 L/s, with three replicates for each rates applied at 1m distance of each other. The experiments were carried out at three sites within one burn perimeter with different burn severities ranging from low to high, with two replicates at each site. Runon was applied until an apparent steady state in runoff was reached at the lower plot boundary, which was typically between 0.7 and 2.5 minutes. The experiments were filmed and erosion depth was measured by survey methods at 1m intervals. Soil surface properties, including potential sediment trapping objects were measured and surveyed near the plots. We found that fire severity increased plot scale average erosion depth significantly even as experiments were typically much shorter on the high severity plots. Unit stream power was a good predictor for average erosion depth. Uncontrolled for variations in soil surface properties explained process behaviour: finer, ash rich surface material was much less likely to be trapped by fallen, charred branches and litter than coarser, ash-depleted material.     Background Spontaneous pneumothorax (SP) is defined by the presence of air in the pleural space without history of trauma. It is classified as secondary if coexisting with underlying pulmonary disease. Its an entity with considerable incidence and treatment particularities which give reason for a reflection on the subject. We present a 5-year casuistry, characterizing the SP epidemiology, clinical presentation, investigation and therapeutic choices. Methods Sixty-six patients were included in the study, corresponding to 93 episodes of SP. Results We have found male predominance and the mean age was 34.5 years old. In 60.6% of cases there was history of tobacco use; 36.4% of cases were classified as secondary; 30.1% of patients with secondary SP and 21.7% with primary SP recurred; 89.2% had an acute presentation. The most frequent initial symptom was chest pain (90.3%) and 81.7% had diminished breath sounds. In 17.3% it was documented a physical strain associated. We did not identify statistically significant association between the SP occurrence and the variation of the atmospheric pressure, on the first day of symptoms. In 12.9% of episodes the initial treatment option was observation. In most of the episodes the lung totally expanded. However, in 29.1% of the episodes surgical treatment was needed. Conclusions Our results are similar to the literature. Some clinical records are incomplete, demanding the implementation of rules to improve knowledge about this matter.     The ultimate objective of the tasks described in the research agreement was to identify resources primarily, but not exclusively, within New York State that are available for the development of a Center for Advanced Microelectronics Processing (CAMP). Identification of those resources would enable Brookhaven National Laboratory to prepare a program plan for the CAMP. In order to achieve the stated goal, the principal investigators undertook to meet the key personnel in relevant NYS industrial and academic organizations to discuss the potential for economic development that could accompany such a Center and to gauge the extent of participation that could be expected from each interested party. Integrated of these discussions was to be achieved through a workshop convened in the summer of 1990. The culmination of this workshop was to be a report (the final report) outlining a plan for implementing a Center in the state. As events unfolded, it became possible to identify the elements of a major center for x-ray lithography on Lone Island at Brookhaven National Laboratory. The principal investigators were than advised to substitute a working document based upon that concept in place of a report based upon the more general CAMP workshop originally envisioned. Following that suggestion from the New York State Science and Technology Foundation, the principals established a working group consisting of representatives of the Grumman Corporation, Columbia University, the State University of New York at Stony Brook, and Brookhaven National Laboratory. Regular meetings and additional communications between these collaborators have produced a preproposal that constitutes the main body of the final report required by the contract. Other components of this final report include the interim report and a brief description of the activities which followed the establishment of the X-ray Lithography Center working group.

    This final rule amends the Medicaid regulations to define and describe state plan section 1915(i) home and community-based services (HCBS) under the Social Security Act (the Act) amended by the Affordable Care Act. This rule offers states new flexibilities in providing necessary and appropriate services to elderly and disabled populations. This rule describes Medicaid coverage of the optional state plan benefit to furnish home and community based-services and draw federal matching funds. This rule also provides for a 5-year duration for certain demonstration projects or waivers at the discretion of the Secretary, when they provide medical assistance for individuals dually eligible for Medicaid and Medicare benefits, includes payment reassignment provisions because state Medicaid programs often operate as the primary or only payer for the class of practitioners that includes HCBS providers, and amends Medicaid regulations to provide home and community-based setting requirements related to the Affordable Care Act for Community First Choice State plan option. This final rule also makes several important changes to the regulations implementing Medicaid 1915(c) HCBS waivers. PMID:24443765

    The NASA Infrared Telescope Facility (IRTF) is a 3-meter optical/IR telescope dedicated to NASA-related programs of mission support and basic solar system research. All of the funding for IRTF operations comes from the Planetary Astronomy Program. We are preparing the Cooperative Agreement with NASA for the next 5 years (Feb. 2006 -- Jan. 2011). We will strive to refurbish the telescope in order to provide mission support and to allow the IRTF to provide fundamental data for future missions to Mars, comets, satellites, Near-Earth Objects, and asteroids. A major component of our activities will be to improve the image quality of the telescope and to provide high dynamic imaging on the IRTF.     In the product-process development closed-loop an integrated product and process plan model is essential for structuring and interchanging data and information. Many of the currently existing standards (STEP) provide an appropriate solution for the different stages of the closed-loop using a clear feature-based approach. However, inspection planning is not undertaken in the same manner and detailed inspection (measurement) planning is performed directly. In order to carry out inspection planning, that is both integrated and at the same level as process planning, the Inspection Feature (InspF) is proposed here, which is directly related with product and process functionality. The proposal includes an InspF library that makes it possible part interpretation from an inspection point of view, while also providing alternatives and not being restricted to the use of just one single type of measurement equipment.

    We present a case of pyomyositis in an otherwise healthy 5-year-old child that underscores the potential for serious, life-threatening complications. Pyomyositis of the gluteal, psoas, and iliacus muscles was associated with osteomyelitis, septic arthritis, a large inferior vena cava thrombus, septic pulmonary emboli, and eventual pneumonia. Primary pyomyositis is a purulent infection of striated muscle thought to be caused by seeding from a transient bacteremia. The focal infection typically forms an abscess that generally responds to intravenous antibiotics and occasionally requires adjunctive computed tomography-guided aspiration and drainage. This localized infectious process rarely produces further sequelae unless treatment is delayed. Pyomyositis is rare in healthy individuals and requires a high clinical suspicion in patients who present with fever, leukocytosis, and localized pain. PMID:10910315

    During the Cassini Equinox Mission, the Optical Navigation strategy has gradually evolved toward maintenance of an acceptable level of uncertainty on the positions of the bodies to be observed. By counteracting the runoff of the uncertainty over time, this strategy helps satisfy the spacecraft pointing requirements throughout the Solstice Mission, while considerably reducing the required imaging frequency. Requirements for planning observations were established, and the planning process itself was largely automated to facilitate re-planning if it becomes necessary.   8. Parallel plan execution with self-processing networks

    A critical issue for space operations is how to develop and apply advanced automation techniques to reduce the cost and complexity of working in space. In this context, it is important to examine how recent advances in self-processing networks can be applied for planning and scheduling tasks. For this reason, the feasibility of applying self-processing network models to a variety of planning and control problems relevant to spacecraft activities is being explored. Goals are to demonstrate that self-processing methods are applicable to these problems, and that MIRRORS/II, a general purpose software environment for implementing self-processing models, is sufficiently robust to support development of a wide range of application prototypes. Using MIRRORS/II and marker passing modelling techniques, a model of the execution of a Spaceworld plan was implemented. This is a simplified model of the Voyager spacecraft which photographed Jupiter, Saturn, and their satellites. It is shown that plan execution, a task usually solved using traditional artificial intelligence (AI) techniques, can be accomplished using a self-processing network. The fact that self-processing networks were applied to other space-related tasks, in addition to the one discussed here, demonstrates the general applicability of this approach to planning and control problems relevant to spacecraft activities. It is also demonstrated that MIRRORS/II is a powerful environment for the development and evaluation of self-processing systems.

    By adopting The NATO Strategic Concept Document in 2010, some important changes in the perception of threat and management of crisis were introduced. This new concept, named ''Comprehensive Approach'', includes the precautions of pre-crisis management, applications of crisis-duration management and reconstruction phase of post-intervention management. NATO will be interested in not only the political and military options , but also social, economical and informational aspects of crisis. NATO will take place in all phases of conflict. The conflicts which occur outside the borders of NATO's nations and terrorism are perceived as threat sources for peace and stability. In addition to conventional threats, cyber attacks which threaten network-supported communication systems, preventing applications from accessing to space that will be used in different fields of life. On the other hand, electronic warfare capabilities which can effect us negatively are added to threat list as new threats. In the process in which military is thought as option, a harder planning phase is waiting for NATO's decision makers who struggle for keeping peace and security. Operation planning process which depends on comprehensive approach, contains these steps: Situational awareness of battlefield, evaluation of the military intervention options, orientation, developing an operation plan, reviewing the plan and transition phases.1 To be successful in theater which is always changing with the technological advances, there has to be an accurate and timely planning on the table.     Large capital building projects benefit from an operational readiness strategy prior to new facility occupancy. St. Joseph's Healthcare used a structured approach for their readiness planning that included individual work plan meetings, tools for ensuring integration across programs and services and process improvement support to ensure a smooth transition. Over 1100 staff were oriented using a Train-the-Trainer model. Significant effort was required to co-ordinate the customized training, which involved "staffing up" to ensure sufficient resources for backfill. Operational readiness planning places additional demands on managers, requiring support and assistance from dedicated resources both prior to occupancy and several months post-move. PMID:25906468

Sequentially, to successfully explore the unknown, there has to be a planning process that organizes certain events in the right priority. Therefore, the planning support team has to continually improve their processes so the ISS Mission Operations can operate smoothly and effectively. The planning support team consists of people in the Long Range Planning area that develop timelines that includes International Partner?s Preliminary STP inputs all the way through to publishing of the Final STP. Planning is a crucial part of the NASA community when it comes to planning the astronaut?s daily schedule in great detail. The STP Process is in need of improvement, because of the various tasks that are required to be broken down in order to get the overall objective of developing a Final STP done correctly. Then a new project came along in order to store various data in a more efficient database.     Conservation Action Planning (CAP) is an adaptive management planning process refined by The Nature Conservancy (TNC) and embraced worldwide as the Open Standards for the Practice of Conservation. The CAP process facilitates open, multi-institutional collaboration on a common conservation agenda through organized actions and quantified results. While specifically designed for conservation efforts, the framework is adaptable and flexible to multiple scales and can be used for any collaborative planning effort. The CAP framework addresses inception; design and development of goals, measures, and strategies; and plan implementation and evaluation. The specific components of the CAP include defining the project scope and conservation targets; assessing the ecological viability; ascertaining threats and surrounding situation; identifying opportunities and designing strategies for action; and implementing actions and monitoring results. In 2007, TNC and a multidisciplinary graduate student team from the University of Michigan's School of Natural Resources and Environment initiated a CAP for the St. Marys River, the connecting channel between Lake Superior and Lake Huron, and its local watershed. The students not only gained experience in conservation planning, but also learned lessons that notably benefited the CAP process and were valuable for any successful collaborative effort—a dedicated core team improved product quality, accelerated the timeline, and provided necessary support for ongoing efforts; an academic approach in preparation for engagement in the planning process brought applicable scientific research to the forefront, enhanced workshop facilitation, and improved stakeholder participation; and early and continuous interactions with regional stakeholders improved cooperation and built a supportive network for collaboration.

The overall plan shows approximate cost of major equipment, production rate and performance capability, flexibility, and special maintenance requirements. Direct computer control is compared with supervisory-mode operations. The plan is limited to wafer processing operations from the starting wafer to the finished beam-lead die after separation etching.     This paper describes the Multimission VICAR (Video Image Communication and Retrieval) Planner (MVP) (Chien 1994) system, which uses artificial intelligence planning techniques (Iwasaki & Friedland, 1985, Pemberthy & Weld, 1992, Stefik, 1981) to automatically construct executable complex image processing procedures (using models of the smaller constituent image processing subprograms) in response to image processing requests made to the JPL Multimission Image Processing Laboratory (MIPL). The MVP system allows the user to specify the image processing requirements in terms of the various types of correction required. Given this information, MVP derives unspecified required processing steps and determines appropriate image processing programs and parameters to achieve the specified image processing goals. This information is output as an executable image processing program which can then be executed to fill the processing request.

    Fish and its processed products are the most affordable source of animal protein in the diet of most people in Indonesia. The goal in production planning is to meet customer demand over a fixed time horizon divided into planning periods by optimizing the trade-off between economic objectives such as production cost and customer satisfaction level. The major decisions are production and inventory levels for each product and the number of workforce in each planning period. In this paper we consider the management of small scale traditional business at North Sumatera Province which performs processing fish into several local seafood products. The inherent uncertainty of data (e.g. demand, fish availability), together with the sequential evolution of data over time leads the production planning problem to a nonlinear mixed-integer stochastic programming model. We use scenario generation based approach and feasible neighborhood search for solving the model. The results which show the amount of each fish processed product and the number of workforce needed in each horizon planning are presented.

    The process of preparing a sample plan for optical and resist model calibration has always been tedious. Not only because it is required to accurately represent full chip designs with countless combinations of widths, spaces and environments, but also because of the constraints imposed by metrology which may result in limiting the number of structures to be measured. Also, there are other limits on the types of these structures, and this is mainly due to the accuracy variation across different types of geometries. For instance, pitch measurements are normally more accurate than corner rounding. Thus, only certain geometrical shapes are mostly considered to create a sample plan. In addition, the time factor is becoming very crucial as we migrate from a technology node to another due to the increase in the number of development and production nodes, and the process is getting more complicated if process window aware models are to be developed in a reasonable time frame, thus there is a need for reliable methods to choose sample plans which also help reduce cycle time. In this context, an automated flow is proposed for sample plan creation. Once the illumination and film stack are defined, all the errors in the input data are fixed and sites are centered. Then, bad sites are excluded. Afterwards, the clean data are reduced based on geometrical resemblance. Also, an editable database of measurement-reliable and critical structures are provided, and their percentage in the final sample plan as well as the total number of 1D/2D samples can be predefined. It has the advantage of eliminating manual selection or filtering techniques, and it provides powerful tools for customizing the final plan, and the time needed to generate these plans is greatly reduced.

    As low income countries experience economic transition, characterized by rapid economic growth and increased government spending potential in health, they have increased fiscal space to support and sustain more of their own health programmes, decreasing need for donor development assistance. Phase out of external funds should be systematic and efforts towards this end should concentrate on government commitments towards country ownership and self-sustainability. The 2006 US Agency for International Development (USAID) family planning (FP) graduation strategy is one such example of a systematic phase-out approach. Triggers for graduation were based on pre-determined criteria and programme indicators. In 2011 the GAVI Alliance (formerly the Global Alliance for Vaccines and Immunizations) which primarily supports financing of new vaccines, established a graduation policy process. Countries whose gross national income per capita exceeds $1570 incrementally increase their co-financing of new vaccines over a 5-year period until they are no longer eligible to apply for new GAVI funding, although previously awarded support will continue. This article compares and contrasts the USAID and GAVI processes to apply lessons learned from the USAID FP graduation experience to the GAVI process. The findings of the review are 3-fold: (1) FP graduation plans served an important purpose by focusing on strategic needs across six graduation plan foci, facilitating graduation with pre-determined financial and technical benchmarks, (2) USAID sought to assure contraceptive security prior to graduation, phasing out of contraceptive donations first before phasing out from technical assistance in other programme areas and (3) USAID sought to sustain political support to assure financing of products and programmes continue after graduation.     In 2006, the SciDAC Scientific Data Management (SDM) Center proposed to continue its work deploying leading edge data management and analysis capabilities to scientific applications. One of three thrust areas within the proposed center was focused on Scientific Process Automation (SPA) using workflow technology. As a founding member of the Kepler consortium [LAB+09], the SDM Center team was well positioned to begin deploying workflows immediately. We were also keenly aware of some of the deficiencies in Kepler when applied to high performance computing workflows, which allowed us to focus our research and development efforts on critical new capabilities which were ultimately integrated into the Kepler open source distribution, benefiting the entire community. Significant work was required to ensure Kepler was capable of supporting large-scale production runs for SciDAC applications. Our work on generic actors and templates have improved the portability of workflows across machines and provided a higher level of abstraction for workflow developers. Fault tolerance and provenance tracking were obvious areas for improvement within Kepler given the longevity and complexity of our target workflows. To monitor workflow execution, we developed and deployed a web-based dashboard. We then generalized this interface and released it so it could be deployed at other locations. Outreach has always been a primary focus of our work and we had many successful deployments across a number of scientific domains while continually publishing and presenting our work. This short paper describes our most significant accomplishments over the past 5 years.   11. A Search Algorithm for Generating Alternative Process Plans in Flexible Manufacturing System

    Capabilities and complexity of manufacturing systems are increasing and striving for an integrated manufacturing environment. Availability of alternative process plans is a key factor for integration of design, process planning and scheduling. This paper describes an algorithm for generation of alternative process plans by extending the existing framework of the process plan networks. A class diagram is introduced for generating process plans and process plan networks from the viewpoint of the integrated process planning and scheduling systems. An incomplete search algorithm is developed for generating and searching the process plan networks. The benefit of this algorithm is that the whole process plan network does not have to be generated before the search algorithm starts. This algorithm is applicable to large and enormous process plan networks and also to search wide areas of the network based on the user requirement. The algorithm can generate alternative process plans and to select a suitable one based on the objective functions.

    This paper describes formulations of the Evaluation Planning Module that have been developed since its inception. This module is one of the core algorithms in the Pantex Process Model, a computerized model to support production planning in a complex manufacturing system at the Pantex Plant, a US Department of Energy facility. Pantex is responsible for three major DOE programs -- nuclear weapons disposal, stockpile evaluation, and stockpile maintenance -- using shared facilities, technicians, and equipment. The model reflects the interactions of scheduling constraints, material flow constraints, and the availability of required technicians and facilities.

    The Breakfast Task (Craik & Bialystok, 2006) is a computerized task that simulates the planning and monitoring requirements involved in cooking breakfast, an everyday activity important for functional independence. In Experiment 1, 28 adults performed the Breakfast Task, and outcome measures were examined with principal component analysis to elucidate the structure of cognitive processes underlying performance. Analyses revealed a 2-component structure which putatively captured global planning and local monitoring abilities. In Experiment 2, the structure of Breakfast Task performance was cross-validated on a new sample of 59 healthy older adults who also performed tests assessing working memory, processing speed, inhibition, reasoning and prospective memory. Factor analyses showed that the global planning component from the Breakfast Task was significantly correlated with individual differences in executive functions but the local monitoring component was independent of such functions. The Breakfast Task provides a fast, enjoyable, and lifelike assessment of complex everyday planning and monitoring, and their underlying processes such as working memory and executive functions.     The upscaling process planning developed in this study primarily involved optimizing operating parameters, i.e., dilution ratios, during process designs. Minimal variable cost was used as an indicator for selecting the optimal combination of dilution ratios. The upper and lower mean confidence intervals obtained from the actual cultured cell density data were used as the final cell density stability indicator after the operating parameters or dilution ratios were selected. The process planning method and results were demonstrated through three case studies of batch culture simulation. They are (1) final objective cell densities were adjusted, (2) high and low light intensities were used for intermediate-scale cultures, and (3) the number of culture days was expressed as integers for the intermediate-scale culture.     Effective use of resources that are shared among multiple products or processes is critical for agile manufacturing. This paper describes the development and implementation of a computerized model to support production planning in a complex manufacturing system at the Pantex Plant, a US Department of Energy facility. The model integrates two different production processes (nuclear weapon disposal and stockpile evaluation) that use common facilities and personnel at the plant. The two production processes are characteristic of flow-shop and job shop operations. The model reflects the interactions of scheduling constraints, material flow constraints, and the availability of required technicians and facilities. Operational results show significant productivity increases from use of the model.

  3. Reasoning about change and exceptions in automated process planning

    Automated process planning is generally defined as the automatic planning of the manufacturing procedures for producing a part from a CAD based product definition. The knowledge in this domain is largely heuristic and has been a good application of expert systems for developing an automated planner. We are currently developing an automated process planning system, XCUT, using the HERB rule-based expert system shell which employs hierarchical abstraction and object-oriented programming. Two areas where we have found the AI techniques implemented in HERB lacking for our domain are reasoning about change and exceptions. To reason about change is the frame problem, where after applying an action the planner must determine what facts are still true. Reasoning about exceptions is determining when general heuristics can be used or not. In AI terms reasoning about exceptions is default reasoning or in terms of ATMS is hypothetical reasoning. The focus of this paper will explore both the need and the ways we plan to augment the XCUT system for reasoning about change and exceptions. 19 refs.

    The use of focus groups to determine patron satisfaction with library resources and services is extensive and well established. This article demonstrates how focus groups can also be used to help shape the future direction of a library as part of the strategic planning process. By responding to questions about their long-term library and information needs, focus group participants at the University of Texas Southwestern Medical Center at Dallas Library contributed an abundance of qualitative patron data that was previously lacking from this process. The selection and recruitment of these patrons is discussed along with the line of questioning used in the various focus group sessions. Of special interest is the way the authors utilized these sessions to mobilize and involve the staff in creating the library's strategic plan. This was accomplished not only by having staff members participate in one of the sessions but also by sharing the project's major findings with them and instructing them in how these findings related to the library's future. The authors' experience demonstrates that focus groups are an effective strategic planning tool for libraries and emphasizes the need to share information broadly, if active involvement of the staff is desired in both the development and implementation of the library's strategic plan. The product allows end users to visualize and organize their manufacturing process plans as they are being conceived; avoid redundant and time consuming geometric construction and calculation; automate geometric reasoning processes, and automate downstream manufacturing applications. The product augments a solid model representationmore » of the part with a set of machining features (e.g., pockets, steps, holes, cutouts). The product also enables end users to interact with a solid model to create process plans. It will automatically recognize, or interactively create and modify surface based machining features (represented by sets of faces on the solid model) and volumetric machining features which are represented by delta volumes (solid bodies representing volumes of material to be removed from the part). The FB-Machining Advisor will generate “in process shapes” that represent the shape of the work piece prior or subsequent to a material removal operation.     Depending on the market, refiner`s plans to produce clean fuels and higher value petrochemicals will weigh heavily on the catalytic reformer`s flexibility. It seems that as soon as a timely article related to catalytic reforming operations is published, a new {open_quotes}boutique{close_quotes} gasoline fuel specification is slapped on to existing fuel standards, affecting reformer operations and processing objectives. Just as importantly, the petrochemical market (such as aromatics) that refiners are targeting, can be very fickle. That`s why process engineers have endeavored to maintain an awareness of the flexibility that technology suppliers are building into modern catalytic reformers.

  9. Modified Mandibulotomy Technique to Reduce Postoperative Complications: 5-Year Results

    Purpose To review the 5-year outcomes of our modified mandibulotomy technique. Retrospective review of a tertiary level oral cancer center. Materials and Methods During a 5-year period, 30 patients who had a uniform surgical technique consisting of a lower lip-splitting, modified stair-step osteotomy with thin saw blade and osteotome after plate-precontouring and combination fixation with monocortical osteosynthesis (miniplate) and bicortical osteosynthesis (maxiplate and bicortical screws), with at least 14 months postoperative follow-up, were selected and reviewed retrospectively. Results There were 8 women and 22 men with an average age of 56.5 years. All the patients involved malignancies were squamous cell carcinoma. The main primary sites of the those who underwent a mandibulotomy were the tonsil, the base of tongue, the oral tongue, the retromolar pad area, and others. Others included buccal cheek, floor of mouth, and soft palate. 23 patients received postoperative radiation therapy, and among whom 8 patients also received chemotherapy. Total four (13%) mandibulotomy-related complications occurred, only two (6.7%) requiring additional operation under general anesthesia. Conclusion Our modified mandibulotomy meets the criteria for an ideal mandibulotomy technique relatively well because it requires no intermaxillary fixation, can precise preserve the occlusion in a precise way, allows early function, requires no secondary procedures, and has few complications.   11. Prone Breast Intensity Modulated Radiation Therapy: 5-Year Results

    Purpose: To report the 5-year results of a technique of prone breast radiation therapy delivered by a regimen of accelerated intensity modulated radiation therapy with a concurrent boost to the tumor bed. Methods and Materials: Between 2003 and 2006, 404 patients with stage I-II breast cancer were prospectively enrolled into 2 consecutive protocols, institutional trials 03-30 and 05-181, that used the same regimen of 40.5 Gy/15 fractions delivered to the index breast over 3 weeks, with a concomitant daily boost to the tumor bed of 0.5 Gy (total dose 48 Gy). All patients were treated after segmental mastectomy and had negative margins and nodal assessment. Patients were set up prone: only if lung or heart volumes were in the field was a supine setup attempted and chosen if found to better spare these organs. Results: Ninety-two percent of patients were treated prone, 8% supine. Seventy-two percent had stage I, 28% stage II invasive breast cancer. In-field lung volume ranged from 0 to 228.27 cm{sup 3}, mean 19.65 cm{sup 3}. In-field heart volume for left breast cancer patients ranged from 0 to 21.24 cm{sup 3}, mean 1.59 cm{sup 3}. There was no heart in the field for right breast cancer patients. At a median follow-up of 5 years, the 5-year cumulative incidence of isolated ipsilateral breast tumor recurrence was 0.82% (95% confidence interval [CI] 0.65%-1.04%). The 5-year cumulative incidence of regional recurrence was 0.53% (95% CI 0.41%-0.69%), and the 5-year overall cumulative death rate was 1.28% (95% CI 0.48%-3.38%). Eighty-two percent (95% CI 77%-85%) of patients judged their final cosmetic result as excellent/good. Conclusions: Prone accelerated intensity modulated radiation therapy with a concomitant boost results in excellent local control and optimal sparing of heart and lung, with good cosmesis.     This Facility Effluent Monitoring Plan (FEMP) has been prepared for the 325 Building Radiochemical Processing Laboratory (RPL) at the Pacific Northwest National Laboratory (PNNL) to meet the requirements in DOE Order 5400.1, ''General Environmental Protection Programs.'' This FEMP has been prepared for the RPL primarily because it has a ''major'' (potential to emit >0.1 mrem/yr) emission point for radionuclide air emissions according to the annual National Emission Standards for Hazardous Air Pollutants (NESHAP) assessment performed. This section summarizes the airborne and liquid effluents and the inventory based NESHAP assessment for the facility. The complete monitoring plan includes characterization of effluent streams, monitoring/sampling design criteria, a description of the monitoring systems and sample analysis, and quality assurance requirements. The RPL at PNNL houses radiochemistry research, radioanalytical service, radiochemical process development, and hazardous and radioactive mixed waste treatment activities. The laboratories and specialized facilities enable work ranging from that with nonradioactive materials to work with picogram to kilogram quantities of fissionable materials and up to megacurie quantities of other radionuclides. The special facilities within the building include two shielded hot-cell areas that provide for process development or analytical chemistry work with highly radioactive materials and a waste treatment facility for processing hazardous, mixed radioactive, low-level radioactive, and transuranic wastes generated by PNNL activities.

    Remedial Actions, including Interim Remedial Activities, often require the use of treatment facilities or stabilization techniques using on-site chemical processes. As such, the 29 CFR 1910.119 Process Safety Management (PSM) of Highly Hazardous Chemicals (PSM Standard) and the USEPA regulations for Risk Management Planning require that these chemicals and their attendant potential hazards be identified. A Hazard and Operation (HAZOP) study, Failure Mode and Effect Analysis (FMEA), Fault Tree Analysis, or equivalent graphic presentation of processes must be completed. These studies form a segment of the Process Hazard Analysis (PHA). HAZOP addresses each system and each element of a system that could deviate from normal operations and thus cause a hazard. A full assessment of each process is produced by looking at the hazards, consequences, causes and personnel protection needed. Many variables must be considered when choosing the appropriate PHA technique including the size of the plant, the number of processes, the types of processes, and the types of chemicals used. A mixture of these techniques may be required to adequately transmit information about the process being evaluated.

The 300 APT are a Resource Conservation and Recovery Act of 1976 (RCRA) regulated unit. The 300 APT are included in the Dangerous Waste Portion of the Resource Conservation and Recovery Act Permit for the Treatment, Storage, and Disposal of Dangerous Waste, Permit No. WA890008967, and are subject to final-status requirements for groundwater monitoring. This document describes a compliance monitoring program for groundwater in the uppermost aquifer system at the 300 APT. This plan describes the 300 APT monitoring network, constituent list, sampling schedule, statistical methods, and sampling and analysis protocols that will be employed for the 300 APT. This plan will be used to meet groundwater monitoring requirements from the time the 300 APT becomes part of the Permit and through the postclosure care period until certification of final closure.

During a 418-day 'human rated' experiment, potato crops grown in the chamber provided the equivalent of a continuous supply of the oxygen for one astronaut, along with 55 percent of that long-duration space flight crew member's caloric food requirements and enough purified water for four astronauts while absorbing their expelled carbon dioxide. The experiment provided data that will help demonstarte the feasibility of the CELSS operating as a bioregenerative life support system for lunar and deep-space missions that can operate independently without the need to carry consumables such as air, water and food, while not requiring the expendable air and water system filters necessary on today's human-piloted spacecraft.' His work is an example of the type of life sciences research that will be conducted at the Space Experiment Research Procession Laboratory (SERPL). The SERPL is a planned 100,000-square-foot laboratory that will provide expanded and upgraded facilities for hosting International Space Station experiment processing. In addition, it will provide better support for other biological and life sciences payload processing at KSC. It will serve as a magnet facility for a planned 400-acre Space Station Commerce Park.

  17. A 5-year retrospective clinical study of the Dentium implants

    PURPOSE The aim of this retrospective study was to evaluate cumulative survival rate (CSR) of Implantium implants followed for 5 years and association between risk factors and the CSR. MATERIALS AND METHODS A total of two hundred forty-nine Implantium Implants System (Dentium, Seoul, Korea) placed in ninety-five patients from 2004 to 2009 were investigated with several identified risk factors (sex, systemic disease, smoking, alchohol, reason of tooth loss, length, arch (maxilla or mandible), replace tooth type (incisor, canine, premolar or molar) Kennedy classification, prosthodontic type, prosthodontic design, opposite dentition, abutment type, occlusal material, occlusal unit, splint to tooth, cantilever, other surgery). Clinical examination (mobility, percussion, screw loosening, discomfort, etc.) and radiographic examination data were collected from patient records including all problems during follow-up period according to protocols described earlier. Life table analysis was undertaken to examine the CSR. Cox regression method was conducted to assess the association between potential risk factors and overall CSR. RESULTS Five of 249 implants were failed. Four of these were lost before loading. The 5-year implant cumulative survival rate was 97.37%. Cox regression analysis demonstrated a significant predictive association between overall CSR and systemic disease, smoking, reason of tooth loss, arch, Kennedy classification and prosthodontic design (P<.05). The screw related complication was rare. Two abutment screw fractures were found. Another complications of prosthetic components were porcelain fracture, resin facing fracture and denture fracture (n=19). CONCLUSION The 5-year CSR of Implantium implants was 97.37%. Implant survival may be dependent upon systemic disease, smoking reason of tooth loss, arch, Kennedy classification and prosthodontic design (P<.05).     The vast majority of circumcisions currently performed in the UK are for phimosis or balanitis and the patients are not looking for the denuded glans appearance of a ritual circumcision. We present a refinement of the sleeve technique of circumcision, which involves Horton's test to define the proximal incision margin, and bipolar electro-dissection. A review of all patients undergoing circumcision at the Wordsley Plastic Surgery Unit, in a 5-year period, has shown this technique to be safe with a haematoma rate of only 1.4%, and an overall complication rate of 3%.     The COmet Nucleus Sounding Experiment by Radio wave Transmission (CONSERT / Rosetta) has been designed to sound the interior of the comet 67P/Churyumov-Gerasimenko. This instrument consists of two parts: one onboard Rosetta and the other one onboard Philae. A good CONSERT science measurement sequence requires joint operations of both spacecrafts in a relevant geometry. The geometric constraints to be fulfilled involve the position and the orientation of both Rosetta and Philae. At the moment of planning the post-landing and long-term science operations for Rosetta instruments, the actual comet shape and the landing location remained largely unknown. In addition, the necessity of combining operations of Rosetta spacecraft and Philae spacecraft makes the planning process for CONSERT particularly complex. In this paper, we present the specific methods and tools we developed, in close collaboration with the mission and the science operation teams for both Rosetta and Philae, to identify, rank and plan the operations for CONSERT science measurements. The presented methods could be applied to other missions involving joint operations between two platforms, on a complex shaped object.

    The purpose of this case report is to present a rare finding of synovial sarcoma in a 5-year-old child. Most soft-tissue masses of the foot are too often presumed to be small and benign; therefore, compared with soft-tissue sarcomas, they are difficult to clinically differentiate and treat. A 5-year-old girl presented with a painful lesion that was diagnosed as synovial sarcoma after an excisional biopsy was performed. This was an unexpected finding of synovial sarcoma involving the tibialis posterior tendon of her right foot. The patient presented with an 8-month history of tenderness and an antalgic gait. We would like to encourage that all soft-tissue tumors of the foot be preoperatively evaluated with the aid of diagnostic imaging so that a well-planned biopsy assessment can be performed, with adequate margins excised.     The Duraloc cup is a frequently used metal-backed, porous-coated, hemispherical, press-fit acetabular component. Published data on loosening rates are contradictory. In this study we investigated migration patterns with computer-assisted Einzel-Bild-Roentgen-Analyse (EBRA) of 67 Duraloc 100 cups. Cup migration and clinical scores were analysed over a 5-year follow-up period. Median total migration of the Duraloc 100 cup was 1.21 mm at 5 years. Seventy-five percent of implants were radiologically stable at 2 years and 90% at 4 years. One cup loosened aseptically at 60 months, requiring revision. Cup diameters > or = 54 mm migrated significantly more than cups < 54 mm in diameter (p = 0.029 at 4 years). There was a significant correlation between high polyethylene wear and further migrating cups within the first post-operative year (p = 0.035 at 12 months). Our analysis revealed significantly higher wear in males (p = 0.029 at 4 years). Radiological loosening at two years could be calculated using receiver-operating characteristic curve analysis, and 1.2 mm as an adequate threshold value (sensitivity = 100%, specificity = 89%).     The objective of this study was to create a 5-year survivorship model to identify key clinical features of cystic fibrosis. Such a model could help researchers and clinicians to evaluate therapies, improve the design of prospective studies, monitor practice patterns, counsel individual patients, and determine the best candidates for lung transplantation. The authors used information from the Cystic Fibrosis Foundation Patient Registry (CFFPR), which has collected longitudinal data on approximately 90% of cystic fibrosis patients diagnosed in the United States since 1986. They developed multivariate logistic regression models by using data on 5,820 patients randomly selected from 11,630 in the CFFPR in 1993. Models were tested for goodness of fit and were validated for the remaining 5,810 patients for 1993. The validated 5-year survivorship model included age, forced expiratory volume in 1 second as a percentage of predicted normal, gender, weight-for-age z score, pancreatic sufficiency, diabetes mellitus, Staphylococcus aureus infection, Burkerholderia cepacia infection, and annual number of acute pulmonary exacerbations. The model provides insights into the complex nature of cystic fibrosis and supplies a rigorous tool for clinical practice and research. PMID:11207152

  6. Improving energy audit process and report outcomes through planning initiatives

    Energy audits and energy models are an important aspect of the retrofit design process, as they provide project teams with an opportunity to evaluate a facilities current building systems' and energy performance. The information collected during an energy audit is typically used to develop an energy model and an energy audit report that are both used to assist in making decisions about the design and implementation of energy conservation measures in a facility. The current lack of energy auditing standards results in a high degree of variability in energy audit outcomes depending on the individual performing the audit. The research presented is based on the conviction that performing an energy audit and producing a value adding energy model for retrofit buildings can benefit from a revised approach. The research was divided into four phases, with the initial three phases consisting of: 1.) process mapping activity - aimed at reducing variability in the energy auditing and energy modeling process. 2.) survey analysis -- To examine the misalignment between how industry members use the top energy modeling tools compared to their intended use as defined by software representatives. 3.) sensitivity analysis -- analysis of the affect key energy modeling inputs are having on energy modeling analysis results. The initial three phases helped define the need for an improved energy audit approach that better aligns data collection with facility owners' needs and priorities. The initial three phases also assisted in the development of a multi-criteria decision support tool that incorporates a House of Quality approach to guide a pre-audit planning activity. For the fourth and final research phase explored the impacts and evaluation methods of a pre-audit planning activity using two comparative energy audits as case studies.   7. Realistic Goals and Processes for Future Space Astronomy Portfolio Planning

    It is generally recognized that international participation and coordination is highly valuable for maximizing the scientific impact of modern space science facilities, as well as for cost-sharing reasons. Indeed, all large space science missions, and most medium and small missions, are international, even if one country or space agency has a clear leadership role and bears most of the development costs. International coordination is a necessary aspect of future mission planning, but how that coordination is done remains debatable. I propose that the community's scientific vision is generally homogeneous enough to permit international coordination of decadal-scale strategic science goals. However, the timing and budget allocation/funding mechanisms of individual countries and/or space agencies are too disparate for effective long-term strategic portfolio planning via a single international process. Rather, I argue that coordinated space mission portfolio planning is a natural consequence of international collaboration on individual strategic missions. I review the process and outcomes of the U.S. 2010 decadal survey in astronomy & astrophysics from the perspective of a government official who helped craft the survey charter and transmitted guidance to the scientific community on behalf of a sponsoring agency (NASA), while continuing to manage the current portfolio that involved ongoing negotiations with other space agencies. I analyze the difficulties associated with projecting long-term budgets, obtaining realistic mission costs (including the additional cost burdens of international partnerships), and developing new (possibly transformational) technologies. Finally, I remark on the future role that privately funded space science missions can have in accomplishing international science community goals.

    NASA s vision for Earth Science is to build a "sensor web"; an adaptive array of heterogeneous satellites and other sensors that will track important events, such as storms, and provide real-time information about the state of the Earth to a wide variety of customers. Achieving his vision will require automation not only in the scheduling of the observations but also in the processing af tee resulting data. Ta address this need, we have developed a planner-based agent to automatically generate and execute data-flow programs to produce the requested data products. Data processing domains are substantially different from other planning domains that have been explored, and this has led us to substantially different choices in terms of representation and algorithms. We discuss some of these differences and discuss the approach we have adopted.

    Bulimia nervosa shows a marked tendency to persist, suggesting that powerful maintaining mechanisms operate. Using data from a prospective, 5-year, study of the natural course of 102 people with bulimia nervosa, the authors sought to identify predictors of persistence and to test specific hypotheses derived from the cognitive-behavioral theory of the persistence of bulimia nervosa. The results of both sets of analyses were consistent with the theory, with the degree of overevaluation of shape and weight and a history of childhood obesity predicting a persistent course. There was also support for the central prediction of the cognitive-behavioral theory. These findings suggest that the mechanisms specified by the theory influence its longer term natural course.   14. Modifiable diarrhoea risk factors in Egyptian children aged <5 years.

    By conducting a case-control study in two university hospitals, we explored the association between modifiable risk behaviours and diarrhoea. Children aged <5 years attending outpatient clinics for diarrhoea were matched by age and sex with controls. Data were collected on family demographics, socioeconomic indicators, and risk behaviour practices. Two rectal swabs and a stool specimen were collected from cases and controls. Samples were cultured for bacterial pathogens using standard techniques and tested by ELISA to detect rotavirus and Cryptosporidium spp. Four hundred cases and controls were enrolled between 2007 and 2009. The strongest independent risk factors for diarrhoea were: presence of another household member with diarrhoea [matched odds ratio (mOR) 4.9, 95% CI 2.8-8.4] in the week preceding the survey, introduction to a new kind of food (mOR 3, 95% CI 1.7-5.4), and the child being cared for outside home (mOR 2.6, 95% CI 1.3-5.2). While these risk factors are not identifiable, in some age groups more easily modifiable risk factors were identified including: having no soap for handwashing (mOR 6.3, 95% CI 1.2-33.9) for children aged 7-12 months, and pacifier use (mOR 1.9, 95% CI 1.0-3.5) in children aged 0-6 months. In total, the findings of this study suggest that community-based interventions to improve practices related to sanitation and hygiene, handwashing and food could be utilized to reduce the burden of diarrhoea in Egyptian children aged <5 years.     To empower governments to formulate rational policies without pressure from any group, and to increase the use of evidence-based decision-making to adapt global recommendations on immunization to their local context, the WHO has recommended on multiple occasions that countries should establish National Immunization Technical Advisory Groups (NITAGs). The World Health Assembly (WHA) reinforced those recommendations in 2012 when Member States endorsed the Decade of Vaccines Global Vaccine Action Plan (GVAP). NITAGs are multidisciplinary groups of national experts responsible for providing independent, evidence-informed advice to health authorities on all policy-related issues for all vaccines across all populations. In 2012, according to the WHO-UNICEF Joint Reporting Form, among 57 countries eligible for immunization program financial support from the GAVI Alliance, only 9 reported having a functional NITAG. Since 2008, the Supporting Independent Immunization and Vaccine Advisory Committees (SIVAC) Initiative (at the Agence de Médecine Préventive or AMP) in close collaboration with the WHO and other partners has been working to accelerate and systematize the establishment of NITAGs in low- and middle-income countries. In addition to providing direct support to countries to establish advisory groups, the initiative also supports existing NITAGs to strengthen their capacity in the use of evidence-based processes for decision-making aligned with international standards. After 5 years of implementation and based on lessons learned, we recommend that future efforts should target both expanding new NITAGs and strengthening existing NITAGs in individual countries, along three strategic lines: (i) reinforce NITAG institutional integration to promote sustainability and credibility, (ii) build technical capacity within NITAG secretariats and evaluate NITAG performance, and (iii) increase networking and regional collaborations.     Several current NASA programs such as the EOSDIS Core System (ECS) have data processing and data management requirements that call for an integrated planning and scheduling capability. In this paper, we describe the experience of applying advanced scheduling technology operationally, in terms of what was accomplished, lessons learned, and what remains to be done in order to achieve similar successes in ECS and other programs. We discuss the importance and benefits of advanced scheduling tools, and our progress toward realizing them, through examples and illustrations based on ECS requirements. The first part of the paper focuses on the Data Archive and Distribution (DADS) V0 Scheduler. We then discuss system integration issues ranging from communication with the scheduler to the monitoring of system events and re-scheduling in response to them. The challenge of adapting the scheduler to domain-specific features and scheduling policies is also considered. Extrapolation to the ECS domain raises issues of integrating scheduling with a product-generation planner (such as PlaSTiC), and implementing conditional planning in an operational system. We conclude by briefly noting ongoing technology development and deployment projects being undertaken by HTC and the ISTB.

    The Payload Changeout Room Inspection and Processing System (PIPS) is a highly redundant manipulator intended for performing tasks in the crowded and sensitive environment of the Space Shuttle Orbiter payload bay. Its dexterity will be exploited to maneuver the end effector in a workspace populated with obstacles. A method is described by which the end effector of a highly redundant manipulator is directed toward a target via a Lyapunov stability function. A cost function is constructed which represents the distance from the manipulator links to obstacles. Obstacles are avoided by causing the vector of joint parameters to move orthogonally to the gradient of the workspace cost function. A C language program implements the algorithm to generate a joint history. The resulting motion is graphically displayed using the Interactive Graphical Robot Instruction Program (IGRIP) produced by Deneb Robotics. The graphical simulation has the potential to be a useful tool in path planning for the PIPS in the Shuttle Payload Bay environment.

This NASA Earth Science Sensorweb has been in operational use since 2003, and has proven reliability of the Sensorweb technologies for robust event detection and autonomous response using space and ground assets. Unique features of the software include flexibility to a range of detection and tasking methods including those that require aggregation of data over spatial and temporal ranges, generality of the response structure to represent and implement a range of response campaigns, and the ability to respond rapidly.

The objective of the present research is to develop an integrated process planning and scheduling system which is applicable to the holonic manufacturing systems. In the previous paper, procedures were proposed to recognize the machining features from the product model. A systematic method is proposed, in this paper, to select suitable machining sequences and sequences of machining equipment, by applying the genetic algorithm (GA) and the dynamic programming (DP) methods.

    Survival time prediction at the time of diagnosis is of great importance to make decisions about treatment and long-term follow-up care. However, predicting the outcome of cancer on the basis of clinical information is a challenging task. We now examined the ability of ten different data mining algorithms (Perceptron, Rule Induction, Support Vector Machine, Linear Regression, Naïve Bayes, Decision Tree, k-nearest Neighbor, Logistic Regression, Neural Network, Random Forest) to predict the dichotomous attribute "5-year-survival" based on seven attributes (sex, UICC-stage, etc.) which are available at the time of diagnosis. For this study we made use of the nationwide German research data set on colon cancer provided by the Robert Koch Institute. To assess the results a comparison between data mining algorithms and physicians' opinions was performed. Therefore, physicians guessed the survival time by leveraging the same seven attributes. The average accuracy of the physicians' opinion was 59%, the average accuracy of the machine learning algorithms was 67.7%. PMID:26152957

  5. Equine-associated maxillofacial injuries: retrospective 5-year analysis.

    We explored the relation between the causes of facial injuries in equestrians and the presence or absence of associated injuries. Over a 5-year period we retrospectively reviewed all patients who presented to the John Hunter Hospital, New South Wales, with facial injuries that had resulted from activity with horses. We analysed the rates of hard and soft tissue injuries, and of associated injuries by sex and mechanism. A total of 85 patients were included (50 female and 35 male) with an age range of 2-88 years. There was a significant difference in the rate of maxillofacial and associated injuries when groups were analysed for sex and mechanism of injury. Facial injuries caused by falling from a horse were more often associated with other injuries in men than in women (p<0.05), and men were 4 times more likely to present with associated injuries than women (OR 3.9; 95% CI 1.1 to 14) We also found significant differences in the rates of facial fracture. Women who had been kicked by a horse were more likely to sustain bony injuries than men (p<0.05). Our data confirm the association between kicks and facial fracture, and this may provide an impetus for the development of appropriate protective equipment. Patients who sustain facial injuries when falling from a horse often present with associated injuries and this has practical implications for clinicians involved in their management. PMID:24168759

    The ultimate objective of the tasks described in the research agreement was to identify resources primarily, but not exclusively, within New York State that are available for the development of a Center for Advanced Microelectronics Processing (CAMP). Identification of those resources would enable Brookhaven National Laboratory to prepare a program plan for the CAMP. In order to achieve the stated goal, the principal investigators undertook to meet the key personnel in relevant NYS industrial and academic organizations to discuss the potential for economic development that could accompany such a Center and to gauge the extent of participation that could be expected from each interested party. Integrated of these discussions was to be achieved through a workshop convened in the summer of 1990. The culmination of this workshop was to be a report (the final report) outlining a plan for implementing a Center in the state. As events unfolded, it became possible to identify the elements of a major center for x-ray lithography on Lone Island at Brookhaven National Laboratory. The principal investigators were than advised to substitute a working document based upon that concept in place of a report based upon the more general CAMP workshop originally envisioned. Following that suggestion from the New York State Science and Technology Foundation, the principals established a working group consisting of representatives of the Grumman Corporation, Columbia University, the State University of New York at Stony Brook, and Brookhaven National Laboratory. Regular meetings and additional communications between these collaborators have produced a preproposal that constitutes the main body of the final report required by the contract. Other components of this final report include the interim report and a brief description of the activities which followed the establishment of the X-ray Lithography Center working group.

  13. Blunt traumatic cardiac rupture. A 5-year experience.

    Blunt traumatic cardiac rupture is associated with a high rate of mortality. A review of the computerized trauma registry (1983 to 1988) identified 32 patients with this injury (ages 19 to 65 years; mean age, 39.5 years; 21 men and 11 women). Twenty-one patients (65.6%) were injured in vehicular crashes, 3 (9.4%) in pedestrian accidents, 3 (9.4%) in motorcycle accidents; 3 (9.4%) sustained crush injury; 1 (3.1%) was injured by a fall; and 1 (3.1%) was kicked in the chest by a horse. Anatomic injuries included right atrial rupture (13[40.6%]), left atrial rupture (8 [25%]), right ventricular rupture (10[31.3%]), left ventricular rupture (4[12.5%]), and rupture of two cardiac chambers (3 [9.4%]). Diagnosis was made by thoracotomy in all 20 patients presenting in cardiac arrest. In the remaining 12 patients, the diagnosis was established in seven by emergency left anterolateral thoracotomy and in five by subxyphoid pericardial window. Seven of these 12 patients (58.3%) had clinical cardiac tamponade and significant upper torso cyanosis. The mean Injury Severity Score (ISS), Trauma Score (TS), and Glasgow Coma Scale (GCS) score were 33.8, 13.2, and 14.3, respectively, among survivors and 51.5, 8.3, and 7.0 for nonsurvivors. The overall mortality rate was 81.3% (26 of 32 patients), the only survivors being those presenting with vital signs (6 of 12 patients [50%]). All patients with rupture of two cardiac chambers or with ventricular rupture died. The mortality rate from myocardial rupture is very high. Rapid prehospital transportation, a high index of suspicion, and prompt surgical intervention contribute to survival in these patients. PMID:2256761

    This document is the FY 1993 report on Hanford Site-specific science and technology (S&T) needs for cleanup of the Site as developed via the Hanford Integrated Planning Process (HIPP). It identifies cleanup problems that lack demonstrated technology solutions and technologies that require additional development. Recommendations are provided regarding allocation of funding to address Hanford`s highest-priority technology improvement needs, technology development needs, and scientific research needs, all compiled from a Sitewide perspective. In the past, the S&T agenda for Hanford Site cleanup was sometimes driven by scientists and technologists, with minimal input from the ``problem owners`` (i.e., Westinghouse Hanford Company [WHC] staff who are responsible for cleanup activities). At other times, the problem-owners made decisions to proceed with cleanup without adequate scientific and technological inputs. Under both of these scenarios, there was no significant stakeholder involvement in the decision-making process. One of the key objectives of HIPP is to develop an understanding of the integrated S&T requirements to support the cleanup mission, (a) as defined by the needs of the problem owners, the values of the stakeholders, and the technology development expertise that exists at Hanford and elsewhere. This requires a periodic, systematic assessment of these needs and values to appropriately define a comprehensive technology development program and a complementary scientific research program. Basic to our success is a methodology that is defensible from a technical perspective and acceptable to the stakeholders.

    Automated process planning is becoming a popular research and development topic in engineering and applied artificial intelligence. It is generally defined as the automatic planning of the manufacturing procedures for producing a part from a CAD based product definition. An automated process planning system, XCUT, is currently being developed using rule-based expert system techniques. XCUT will generate process plans for the production of machined piece-parts, given a geometric description of a part's features. The system currently is focused on operation planning for prismatic parts on multi-axis CNC milling machines. To date, moderately complex 2-1/2D prismatic parts have successfully been planned for with approximately 300 rules in the knowledge base. This paper will describe the XCUT system, system architecture, knowledge representation, plan development sequence, and issues in applying expert system technology to automated process planning. 16 refs.

    The Southern Region of the American Burn Association began to craft a regional plan to address a surge of burn-injured patients after a mass casualty event in 2004. Published in 2006, this plan has been tested through modeling, exercise, and actual events. This article focuses on the process of how the plan was created, how it was tested, and how it interfaces with other ongoing efforts on preparedness. One key to success regarding how people respond to a disaster can be traced to preexisting relationships and collaborations. These activities would include training or working together and building trust long before the crisis. Knowing who you can call and rely on when you need help, within the context of your plan, can be pivotal in successfully managing a disaster. This article describes how a coalition of burn center leaders came together. Their ongoing personal association has facilitated the development of planning activities and has kept the process dynamic. This article also includes several of the building blocks for developing a plan from creation to composition, implementation, and testing. The plan discussed here is an example of linking leadership, relationships, process, and documentation together. On the basis of these experiences, the authors believe these elements are present in other regions. The intent of this work is to share an experience and to offer it as a guide to aid others in their regional burn disaster planning efforts. PMID:23666386

    Western Energy Company (WECO) was selected by the Department of Energy (DOE) to demonstrate the Advanced Coal Conversion Process (ACCP) which upgrades low rank coals into high Btu, low sulfur, synthetic bituminous coal. As specified in the Corporate Agreement, RSCP is required to develop an Environmental Monitoring Plan (EMP) which describes in detail the environmental monitoring activities to be performed during the project execution. The purpose of the EMP is to: (1) identify monitoring activities that will be undertaken to show compliance to applicable regulations, (2) confirm the specific environmental impacts predicted in the National Environmental Policy Act documentation, and (3) establish an information base of the assessment of the environmental performance of the technology demonstrated by the project. The EMP specifies the streams to be monitored (e.g. gaseous, aqueous, and solid waste), the parameters to be measured (e.g. temperature, pressure, flow rate), and the species to be analyzed (e.g. sulfur compounds, nitrogen compounds, trace elements) as well as human health and safety exposure levels. The operation and frequency of the monitoring activities is specified, as well as the timing for the monitoring activities related to project phase (e.g. preconstruction, construction, commissioning, operational, post-operational). The EMP is designed to assess the environmental impacts and the environmental improvements resulting from construction and operation of the project.

  20. Clinical spectrum and outcome of pulmonary nocardiosis: 5-year experience

    Background: Pulmonary nocardiosis is a rare but a life-threatening infection caused by Nocardia spp. The diagnosis is often missed and delayed resulting in delay in appropriate treatment and thus higher mortality. Aim: In this study, we aim to evaluate the clinical spectrum and outcome of patients with pulmonary nocardiosis. Methods: A retrospective, 5-year (2009–2014) review of demographic profile, risk factors, clinical manifestations, imaging findings, treatment, and outcome of patients with pulmonary nocardiosis admitted to a tertiary care hospital. Results: The median age of the study subjects was 54 years (range, 16–76) and majority of them (75%) were males. The risk factors for pulmonary nocardiosis identified in our study were long-term steroid use (55.6%), chronic lung disease (52.8%), diabetes (27.8%), and solid-organ transplantation (22.2%). All the patients were symptomatic, and the most common symptoms were cough (91.7%), fever (78%), and expectoration (72%). Almost two-third of the patients were initially misdiagnosed and the alternative diagnosis included pulmonary tuberculosis (n = 7), community-acquired pneumonia (n = 5), lung abscess (n = 4), invasive fungal infection (n = 3), lung cancer (n = 2), and Wegener's granulomatosis (n = 2). The most common radiographic features were consolidation (77.8%) and nodules (56%). The mortality rate for indoor patients was 33% despite treatment. Higher mortality rate was observed among those who had brain abscess (100.0%), HIV positivity (100%), need for mechanical ventilation (87.5%), solid-organ transplantation (50%), and elderly (age > 60 years) patients (43%). Conclusion: The diagnosis of pulmonary nocardiosis is often missed and delayed resulting in delay in appropriate treatment and thus high mortality. A lower threshold for diagnosing pulmonary nocardiosis needs to be exercised, in chest symptomatic patients with underlying chronic lung diseases or systemic immunosuppression, for the early diagnosis

  1. A 5-year experience with an elective scholarly concentrations program

    Problem Programs that encourage scholarly activities beyond the core curriculum and traditional biomedical research are now commonplace among US medical schools. Few studies have generated outcome data for these programs. The goal of the present study was to address this gap. Intervention The Scholarly Concentration (SC) Program, established in 2006 at the Warren Alpert Medical School of Brown University, is a 4-year elective program that not only encourages students to pursue scholarly work that may include traditional biomedical research but also seeks to broaden students’ focus to include less traditional areas. We compared characteristics and academic performance of SC students and non-SC students for the graduating classes of 2010–2014. Context Approximately one-third of our students opt to complete an SC during their 4-year undergraduate medical education. Because this program is additional to the regular MD curriculum, we sought to investigate whether SC students sustained the academic achievement of non-SC students while at the same time producing scholarly work as part of the program. Outcome Over 5 years, 35% of students elected to enter the program and approximately 81% of these students completed the program. The parameters that were similar for both SC and non-SC students were age at matriculation, admission route, proportion of undergraduate science majors, and number of undergraduate science courses. Most academic indicators, including United States Medical Licensing Examinations scores, were similar for the two groups; however, SC students achieved more honors in the six core clerkships and were more likely to be inducted into the medical school's two honor societies. Residency specialties selected by graduates in the two groups were similar. SC students published an average of 1.3 peer-reviewed manuscripts per student, higher than the 0.8 manuscripts per non-SC student (p=0.013).   7. Fasting Plasma Insulin at 5 Years of Age Predicted Subsequent Weight Increase in Early Childhood over a 5-Year Period—The Da Qing Children Cohort Study

    Background The association between hyperinsulinemia and obesity is well known. However, it is uncertain especially in childhood obesity, if initial fasting hyperinsulinemia predicts obesity, or obesity leads to hyperinsulinemia through insulin resistance. Objective To investigate the predictive effect of fasting plasma insulin on subsequent weight change after a 5-year interval in childhood. Methods 424 Children from Da Qing city, China, were recruited at 5 years of age and followed up for 5 years. Blood pressure, anthropometric measurements, fasting plasma insulin, glucose and triglycerides were measured at baseline and 5 years later. Results Fasting plasma insulin at 5 years of age was significantly correlated with change of weight from 5 to 10 years (ΔWeight). Children in the lowest insulin quartile had ΔWeight of 13.08±0.73 kg compare to 18.39±0.86 in the highest insulin quartile (P<0.0001) in boys, and similarly 12.03±0.71 vs 15.80±0.60 kg (P<0.0001) in girls. Multivariate analysis showed that the predictive effect of insulin at 5 years of age on subsequent weight gain over 5 years remained statistically significant even after the adjustment for age, sex, birth weight, TV-viewing time and weight (or body mass index) at baseline. By contrast, the initial weight at 5 years of age did not predict subsequent changes in insulin level 5 years later. Children who had both higher fasting insulin and weight at 5 years of age showed much higher levels of systolic blood pressures, fasting plasma glucose, the homeostasis model assessment for insulin resistance (HOMA-IR) and triglycerides at 10 years of age. Conclusions Fasting plasma insulin at 5 years of age predicts weight gain and cardiovascular risk factors 5 year later in Chinese children of early childhood, but the absolute weight at 5 years of age did not predict subsequent change in fasting insulin.     The Vital Issues process (VIp) is a strategic planning tool initially developed by Sandia National Laboratories (SNL) for the Office of Foreign Intelligence (OFI)* of the US Department of Energy (DOE). It was further developed and refined through its application to a variety of strategic purposes for a range of public and semipublic organizations. The VIp provides a structured mechanism for assisting organizations in accomplishing specified objectives by identifying and prioritizing a portfolio of strategic issues, programmatic areas, or responses to a specified problem. It employs day-long panel meetings in a specified format to elicit a broad range of perspectives on a particular issue in a nonconfrontational manner and to facilitate the interaction and synthesis of diverse viewpoints on a specific topic. The VIp is unique in its incorporation of two primary approaches in each panel session: a qualitative or transactional segment, which entails the synthesis of the alternatives through negotiations or discussion, and a quantitative or net benefit maximization segment, an analytical approach, which involves prioritization of the alternatives using pairwise comparisons. This combination of facilitated group discussion and quantitative ranking provides input to strategic management decisions in the form of stakeholder-defined and -prioritized items as well as information on potential barriers to the implementation of policies and programs. This is the final volume in the series Identifying Vital Issues: New Intelligence Strategies for a New World, a three-volume set that gives an accounting of the VIp as implemented for OFI.     Adults' expertise in face recognition has been attributed to norm-based coding. Moreover, adults possess separable norms for a variety of face categories (e.g., race, sex, age) that appear to enhance recognition by reducing redundancy in the information shared by faces and ensuring that only relevant dimensions are used to encode faces from a given category. Although 5-year-old children process own-race faces using norm-based coding, little is known about the organization and refinement of their face space. The current study investigated whether 5-year-olds rely on category-specific norms and whether experience facilitates the development of dissociable face prototypes. In Experiment 1, we examined whether Chinese 5-year-olds show race-contingent opposing aftereffects and the extent to which aftereffects transfer across face race among Caucasian and Chinese 5-year-olds. Both participant races showed partial transfer of aftereffects across face race; however, there was no evidence for race-contingent opposing aftereffects. To examine whether experience facilitates the development of category-specific prototypes, we investigated whether race-contingent aftereffects are present among Caucasian 5-year-olds with abundant exposure to Chinese faces (Experiment 2) and then tested separate groups of 5-year-olds with two other categories with which they have considerable experience: sex (male/female faces) and age (adult/child faces) (Experiment 3). Across all three categories, 5-year-olds showed no category-contingent opposing aftereffects. These results demonstrate that 5 years of age is a stage characterized by minimal separation in the norms and associated coding dimensions used for faces from different categories and suggest that refinement of the mechanisms that underlie expert face processing occurs throughout childhood.     A lot has happened in a little more than 2 years, and we want give AGU members an update on how things are working under AGU's strategic plan and governance model. AGU is an organization committed to its strategic plan (http://www.agu.org/about/strategic_plan.shtml), and if you have not read the plan lately, we encourage you to do so. AGU's vision is to be an organization that "galvanizes a community of Earth and space scientists that collaboratively advances and communicates science and its power to ensure a sustainable future." We are excited about the progress we have made under this plan and the future course we have set for the Union. Everything the Board of Directors, Council, and committees put on their agendas is intended to advance AGU's strategic goals and objectives. Together with headquarters staff, these bodies are working in an integrated, effective manner to carry out this plan. The best way to demonstrate the progress made and each group's role is to walk through a recent example: the creation of a new Union-level award (see Figure 1).

    Routine HIV testing in primary care settings is now recommended in the United States. The US Department of Veterans Affairs (VA) has increased the number of patients tested for HIV, but overall HIV testing rates in VA remain low. A proven strategy for increasing such testing involves nurse-initiated HIV rapid testing (HIV RT). The purpose of this work was to use a mixed methodology approach to evaluate the 5-year sustainability of an intervention that implemented HIV RT in a VA emergency department setting in a large, urban VA medical center to reduce missed diagnostic and treatment opportunities in this vulnerable patient population. In-person semistructured interviews were conducted with providers and stakeholders. Interview notes were qualitatively coded for emerging themes. Quarterly testing rates were evaluated for a 5-year time span starting from the launch in July 2008. Findings indicate that HIV RT was sustained by the enthusiasm of 2 clinical champions who oversaw the registered nurses responsible for conducting the testing. The departure of the clinical champions was correlated with a substantial drop-off in testing. Findings also indicate potential strategies for improving sustainability including engaging senior leadership in the project, engaging line staff in the implementation planning from the start to increase ownership over the innovation, incorporating information into initial training explaining the importance of the innovation to quality patient care, providing ongoing training to maintain skills, and providing routine progress reports to staff to demonstrate the ongoing impact of their efforts. METHODS This cross-sectional household census survey evaluated 1,209 children < 5 years old living in Bom Jesus, Angola, in 2010. Data were obtained from interviews, questionnaires, child immunization histories, and maternal health histories. The statistical analysis used generalized linear models, in which the dependent variable followed a binary distribution (vaccinated, unvaccinated) and the association function was logarithmic and had the children’s individual, familial, and socioeconomic factors as independent variables. RESULTS Vaccination coverage was 37.0%, higher in children < 1 year (55.0%) and heterogeneous across neighborhoods; 52.0% of children of both sexes had no immunization records. The prevalence rate of vaccination significantly varied according to child age, mother’s level of education, family size, ownership of household appliances, and destination of domestic waste. CONCLUSIONS Vulnerable groups with vaccination coverage below recommended levels continue to be present. Some factors indicate inequalities that represent barriers to full immunization, indicating the need to implement more equitable policies.     Forty-five participants described and rated two events each week during their first term at university. After 3.5 years, we examined whether event characteristics rated in the diary predicted remembering, reliving, and life story importance at the follow-up. In addition, we examined whether ratings of life story importance were consistent across a three year interval. Approximately 60% of events were remembered, but only 20% of these were considered above medium importance to life stories. Higher unusualness, rehearsal, and planning predicted whether an event was remembered 3.5 years later. Higher goal-relevance, importance, emotional intensity, and planning predicted life story importance 3.5 years later. There was a moderate correlation between life story importance rated three months after the diary and rated at the 3.5 year follow-up. The results suggest that autobiographical memory and life stories are governed by different mechanisms and that life story memories are characterized by some degree of stability. PMID:26164104

    The National Nuclear Security Administration s project for developing a unique identifier and a concept for a global monitoring system for UF6 cylinders made significant progress on developing functional requirements and a concept of operation for such a system. The multi-laboratory team is working to define the functional requirements for both the unique identifier and the global monitoring system and to develop a preliminary concept of operations to discuss with key industry stakeholders. Team members began meeting with industry representatives in January 2013 to discuss the preliminary concept and solicit feedback and suggestions. The team has met with representatives from United States Enrichment Corporation, Cameco, URENCO, Honeywell/ConverDyn, and others. This paper presents an overview of the preliminary concept of operations and shares the feedback obtained from the industry engagement meetings.
