The use of expert systems in nuclear power plants to provide advice to managers, supervisors and/or operators is a concept that is rapidly gaining acceptance. f2 Generally, expert systems rely on the expertise of human experts or knowledge that has been codified in publications, books, or regulations to provide advice under a wide variety of conditions. In this work, a probabilistic risk assessment (PRA)3 of a nuclear power plant performed previously is used to assess the safety status of nuclear power plants and to make recommendations to the plant personnel. Nuclear power plants have many redundant systems and can continue to operate when one or more of these systems is disabled or removed from service for maintenance or testing. PRAs provide a means of evaluating the risk to the public associated with the operation of nuclear power plants with components or systems out of service. While the choice of the "source term" and methodology in a PRA may influence the absolute probability and consequences of a core melt, the ratio of two PRA calculations for two configurations of the same plant, carried out on a consistent basis, can readily identify the increase in risk associated with going from one configuration to the other. PRISIM,4 a personal computer program to calculate the ratio of core melt probabilities described above (based on previously performed PRAs), has been developed under the sponsorship of the U.S. Nuclear Regulatory Commission (NRC). When one or several components are removed from service, PRISM then calculates the ratio of the core melt probabilities. The inference engine of the expert system then uses this ratio and a constant risk criterion,5 along with information from its knowledge base (which includes information from the PRA), to advise plant personnel as to what action, if any, should be taken.

Females currently undertaking STEM-related programs can benefit from knowing about how other females who had been in a similar position as them were able to persevere through the challenges of higher education with the help of advisement and student support services that aim to increasing student retention. While there have been a depth of studies on the development of academic advising, there have been limited studies on this development with respect to the needs of specific marginalized groups. This is the gap in literature that is addressed by this study. The outcomes observed in this study can potentially benefit female students at the institution where the study was conducted. This study focused on the group of female students who were able to successfully complete their STEM-related degrees. A significant difference was found between tutoring and learning support, F = 4.65, sd = .78 and a sig. level = .004. A strong negative relationship existed between the ages of the graduates and assessed academic advisement. A perfect positive relationship existed between the age of the graduates and assessed course concierge service scores; and between the age of the graduates and assessed career services and counseling scores. A moderate negative relationship existed between the age of the graduates and assessed curriculum/degree planning database scores, the age of the graduates and assessed academic and program advisement scores and the age of the graduates and assessed tutorial and learning support services scores. A weak negative relationship existed between the age of the graduates and assessed retention scores.

The Environmental Assessment Technical Handbook has four types of Technical Assistance Documents (Sections 2.00-5.00) to assist persons preparing environmental information under 21 CFR Part 25, FDA's policies and procedures for implementing the National Environmental Policy Act. Section 2.00 describes how to assemble environmental information, determine if additional testing is needed, and prepare environmental documents to support petitions and applications for proposed actions. Section 3.00 describes methods used to predict the environmental fate of chemicals: water solubility; n-octanol/water partition coefficient vapor pressure; dissociation constant; ultraviolet-visible absorption spectrum; melting temperature; density and relative density; sorption/desorption; hydrolysis; photodegradation; and biodegradation. Section 4.00 describes methods used to predict the environmental effects of chemicals: algal growth; microbial growth inhibition; seed germination and root elongation; seedling growth; acute toxicity to Daphnia, Hyalella azteca, and freshwater fish; earthworm subacute toxicity; and Daphnia chronic toxicity. Section 5.00 describes procedures used to analyze effects test data. The FDA Center for Food Safety and Applied Nutrition and the FDA Center for Veterinary Medicine prepared the Handbook and will issue revisions or supplements as necessary to reflect comments received and advances in environmental assessment procedures and test methodology.

Human sleep depth was traditionally assessed by scoring electro-encephalographic slow-wave amplitudes at the globally standardized C4-M1 electrode derivation. Since 2007, the American Association of Sleep Medicine (AASM) has accepted three additional derivations for the same purpose. These might well differ in slow wave amplitudes which would bias the scorings. Some derivations might also introduce large inter-individual variability. We compared mean and variability of slow wave amplitudes between six derivations including the four AASM ones. Slow wave amplitudes in those derivations were simultaneously measured using automated analysis in 29 patients. Each amplitude was divided by the average from the six derivations, thus removing shared factors such as age, gender and sleep depth while retaining factors that differ between the derivations such as caused by local skull characteristics, electrode distance and neuronal dipole orientation. The remaining inter-individual variability differed significantly and up to a factor of two between the AASM derivations. The amplitudes differed significantly and up to 60% between the AASM derivations, causing substantial scoring bias between centres using different derivations. The resulting de-standardization most likely affects any patient group because the amplitude differences were consistent over diagnoses, genders, and age. Derivation-dependent amplitude thresholds were proposed to reduce the scoring bias. However, it would be better to settle on just one derivation, for instance Cz-Oz or Fpz-Cz because these have lowest variability while matching the traditional C4-M1 amplitudes. PMID:23940727

Dietary intake provides valuable insights for mounting intervention programs for prevention of disease. With growing concern for adolescent obesity, the need to accurately measure diet becomes imperative. Assessment among adolescents is problematic as this group has irregular eating patterns and have less enthusiasm for recording food intake. Preliminary studies among adolescents suggest that innovative use of technology may improve the accuracy of diet information from young people. In this paper, we propose a novel food record method using a mobile device that will provide an accurate account of daily food and nutrient intake among adolescents. Our approach includes the use of image analysis tools for identification and quantification of food consumption. Images obtained before and after food is consumed can be used to estimate the diet of an individual. In this paper we describe our initial results and indicate the potential of the proposed system.

While many factors depend on successful implantation and outcome of left ventricular assist devices (LVAD), echocardiography remains an integral part and is vital to the success of this process. Transesophageal echocardiography (TEE) allows interrogation of all the cardiac structures and great vessels. The pre-implantation TEE exam establishes a baseline and may identify potential problems that need palliation. Among these, most significant are aortic insufficiency (AI), intracardiac thrombi, poor right ventricular (RV) function, and intracardiac shunts. The post-implantation exam allows for adequate de-airing of the heart and successful LVAD initiation. The position and flow profiles of the inflow and outflow cannulas of the LVAD may be assessed. Finally, it assists in the astute management and vigilant identification and correction of a number of complications in the immediate post-implantation period. A probabilistic risk assessment (PRA) approach has been developed and applied to the risk analysis of capsule abort during ascent. The PRA is used to assist in the identification of modeling and simulation applications that can significantly impact the understanding of crew risk during this potentially dangerous maneuver. The PRA approach is also being used to identify the appropriate level of fidelity for the modeling of those critical failure modes. The Apollo launch escape system (LES) was chosen as a test problem for application of this approach. Failure modes that have been modeled and/or simulated to date include explosive overpressure-based failure, explosive fragment-based failure, land landing failures (range limits exceeded either near launch or Mode III trajectories ending on the African continent), capsule-booster re-contact during separation, and failure due to plume-induced instability. These failure modes have been investigated using analysis tools in a variety of technical disciplines at various levels of fidelity. The current paper focuses on the development and application of a blast overpressure model for the prediction of structural failure due to overpressure, including the application of high-fidelity analysis to predict near-field and headwinds effects.

In general the software required by a blast designer includes tools that graphically present blast designs (surface and underground), can analyze a design or predict its result, and can assess blasting results. As computers develop and computer literacy continues to rise the development of and use of such tools will spread. An example of the tools that are becoming available includes: Automatic blast pattern generation and underground ring design; blast design evaluation in terms of explosive distribution and detonation simulation; fragmentation prediction; blast vibration prediction and minimization; blast monitoring for assessment of dynamic performance; vibration measurement, display and signal processing; evaluation of blast results in terms of fragmentation; and risk and reliability based blast assessment. The authors have identified a set of criteria that are essential in choosing appropriate software blasting tools.

The United States Department of Energy Office of Environmental Management (DOE-EM) has established a Performance Assessment Community of Practice (PA CoP) to foster the sharing of information among performance assessment (PA) and risk assessment practitioners, regulators and oversight personnel. The general intent is to contribute to continuous improvement in the consistency, technical adequacy and quality of implementation of PAs and risk assessments around the DOE Complex. The PA CoP activities have involved commercial disposal facilities and international participants to provide a global perspective. The PA CoP has also sponsored annual technical exchanges as a means to foster improved communication and to share lessons learned from on-going modelling activities. The PA CoP encourages activities to provide programmatic and technical assistance in the form of sharing experience and lessons learned with practitioners during the development of PAs and risk assessments. This assistance complements DOE-EM reviews through the Low-Level Waste Disposal Facility Federal Review Group (LFRG) that are conducted after modelling efforts are completed. Such up-front assistance is providing additional value in terms of improving consistency and sharing of information. There has been a substantial increase in the amount of assistance being provided. The assistance has been well received by practitioners and regulators that have been involved. We are developing methods to identify food items using a single image acquired from the mobile device. Our goal is to automatically determine the regions in an image where a particular food is located (segmentation) and correctly identify the food type based on its features (classification or food labeling). Robot-mediated therapy (RMT) has been a very dynamic area of research in recent years. Robotics devices are in fact capable to quantify the performances of a rehabilitation task in treatments of several disorders of the arm and the shoulder of various central and peripheral etiology. Different systems for robot-aided neuro-rehabilitation are available for upper limb rehabilitation but the biomechanical parameters proposed until today, to evaluate the quality of the movement, are related to the specific robot used and to the type of exercise performed. Besides, none study indicated a standardized quantitative evaluation of robot assisted upper arm reaching movements, so the RMT is still far to be considered a standardised tool. In this paper a quantitative kinematic assessment of robot assisted upper arm reaching movements, considering also the effect of gravity on the quality of the movements, is proposed. We studied a group of 10 healthy subjects and results indicate that our advised protocol can be useful for characterising normal pattern in reaching movements. PMID:27459844

Comprehensive measurements of the condition of provided care and the activities of daily living of the subjects, which are included in the HCS, are required for the prediction of advisability of home care. Overwhelming intrusion alerts have made timely response to network security breaches a difficult task. Correlating alerts to produce a higher level view of intrusion state of a network, thus, becomes an essential element in network defense. This work proposes to analyze correlated or grouped alerts and determine their 'impact' to services and users of the network. A network is modeled as 'virtual terrain' where cyber attacks maneuver. Overlaying correlated attack tracks on virtual terrain exhibits the vulnerabilities exploited by each track and the relationships between them and different network entities. The proposed impact assessment algorithm utilizes the graph-based virtual terrain model and combines assessments of damages caused by the attacks. The combined impact scores allow to identify severely damaged network services and affected users. Several scenarios are examined to demonstrate the uses of the proposed Virtual Terrain Assisted Impact Assessment for Cyber Attacks (VTAC).

In order to facilitate systematic, computer aided improvements of camouflage and concealment assessment methods, the software system CART (Camouflage Assessment in Real-Time) was built up for the camouflage assessment of objects in multispectral image sequences (see contributions to SPIE 2007, SPIE 2008 and SPIE 2009 , , ). It comprises a semi-automatic marking of target objects (ground truth generation) including their propagation over the image sequence and the evaluation via user-defined feature extractors. The conspicuity of camouflaged objects due to their movement can be assessed with a purpose-built processing method named MTI snail track algorithm. This paper presents the enhancements over the recent year and addresses procedures to assist the camouflage assessment of moving objects for image data material with strong noise or image artefacts. This extends the evaluation methods significantly to a broader application range. For example, some noisy infrared image data material can be evaluated for the first time by applying the presented methods which fathom the correlations between camouflage assessment, MTI (moving target indication) and dedicated noise filtering.

Findings in behavioral science, including psychology, have influenced policies and reforms in many nations. Choice architecture can affect outcomes even if material incentives are not involved. In some contexts, default rules, simplification, and social norms have had even larger effects than significant economic incentives. Psychological research is helping to inform initiatives in savings, finance, highway safety, consumer protection, energy, climate change, obesity, education, poverty, development, crime, corruption, health, and the environment. No nation has yet created a council of psychological advisers, but the role of behavioral research in policy domains is likely to grow in the coming years, especially in light of the mounting interest in promoting ease and simplification ("navigability"); in increasing effectiveness, economic growth, and competitiveness; and in providing low-cost, choice-preserving approaches. Self-assessment of classroom assistive listening devices (ALDs) based on induction loop systems was carried out in Swedish classes for hearing-impaired students. A questionnaire was developed and completed by 25 students (bilateral hearing aid users, 10-20 years old). Responses for hearing aid microphone mode (M) and telecoil mode (T) were collected. Two attributes, audibility and awareness, were identified and assigned to either mode. Better audibility was achieved in T-mode. Students with severe hearing loss benefited more using T-mode when compared to the better hearing students, especially in more difficult listening situations. Better awareness was achieved in M-mode; students could better hear, locate and segregate sounds in the environment around them. Depending on the situation, students make different choices between audibility and awareness. Self-assessment is a promising approach for determining what combination of ALD design and function that will best benefit the students. PMID:20524774

To reduce the risk of extinction due to climate change, some ecologists have suggested human-aided translocation of species, or assisted migration (AM), to areas where climate is projected to become suitable. Such intentional movement, however, may create new invasive species if successful introductions grow out of control and cause ecologic or economic damage. We assessed this risk by surveying invasive species in the United States and categorizing invaders based on origin. Because AM will involve moving species on a regional scale within continents (i.e., range shifts), we used invasive species with an intracontinental origin as a proxy for species that would be moved through AM. We then determined whether intracontinental invasions were more prevalent or harmful than intercontinental invasions. Intracontinental invasions occurred far less frequently than invasions from other continents, but they were just as likely to have had severe effects. Fish and crustaceans pose a particularly high threat of intracontinental invasion. We conclude that the risk of AM to create novel invasive species is small, but assisted species that do become invasive could have large effects. Past experience with species reintroductions may help inform policy regarding AM. PMID:18577085

Radiofrequency ablation (RFA) is becoming a standard procedure for minimally invasive tumor treatment in clinical practice. Due to its common technical procedure, low complication rate, and low cost, RFA has become an alternative to surgical resection in the liver. To evaluate the therapy success of RFA, thorough follow-up imaging is essential. Conventionally, shape, size, and position of tumor and coagulation are visually compared in a side-by-side manner using pre- and post-interventional images. To objectify the verification of the treatment success, a novel software assistant allowing for fast and accurate comparison of tumor and coagulation is proposed. In this work, the clinical value of the proposed assessment software is evaluated. In a retrospective clinical study, 39 cases of hepatic tumor ablation are evaluated using the prototype software and conventional image comparison by four radiologists with different levels of experience. The cases are randomized and evaluated in two sessions to avoid any recall-bias. Self-confidence of correct diagnosis (local recurrence vs. no local recurrence) on a six-point scale is given for each case by the radiologists. Sensitivity, specificity, positive and negative predictive values as well as receiver operating curves are calculated for both methods. It is shown that the software-assisted method allows physicians to correctly identify local tumor recurrence with a higher percentage than the conventional method (sensitivity: 0.6 vs. 0.35), whereas the percentage of correctly identified successful ablations is slightly reduced (specificity: 0.83 vs. 0.89).

Although biodiesel can be intentionally tampered with, impairing its quality, ineffective production processes may also result in a nonconforming final fuel. For an incomplete transesterification reaction, traces of alcohol (ethanol or methanol) or remaining raw material (vegetable oil or animal fats) may be harmful to consumers, the environment or to engines. Traditional methods for biodiesel assessment are complex, time consuming and expensive, leading to the need for the development of new and more versatile processes for quality control. This work describes a refractometric fibre optic based sensor that is thermally assisted, developed to quantify the remaining methanol or vegetable oil in biodiesel blends. The sensing relies on a long period grating to configure an in-fibre interferometer. A complete analytical routine is demonstrated for the sensor allowing the evaluation of the biodiesel blends without segregation of the components. The results show the sensor can determine the presence of oil or methanol in biodiesel with a concentration ranging from 0% to 10% v/v. The sensor presented a resolution and standard combined uncertainty of 0.013% v/v and 0.62% v/v for biodiesel-oil samples, and 0.007% v/v and 0.22% v/v for biodiesel-methanol samples, respectively.

Purpose To examine the relationship between cognitive status and the usability of an integrated medication delivery unit (MDU) in older adults who reside in an Assisted Living Facility (ALF). Methods Subjects were recruited from a single ALF in Pittsburgh, PA. Usability testing sessions required subjects to execute tasks essential to using EMMA® (Electronic Medication Management Assistant), a Class II Federal Drug Administration (FDA) approved integrated MDU. Video coding allowed for quantification of usability errors observed during the testing sessions. Each subject's cognitive status was assessed using the Mini Mental State Exam (MMSE®) with scores <24 indicating cognitive impairment. Functional status was assessed using the Lawton Instrumental Activities of Daily Living (IADL) questionnaire, and a global assessment of subjective usability was assessed by completing the System Usability Scale (SUS). Non-parametric statistics and correlation analysis were used to determine whether significant differences existed between cognitively impaired and non-impaired subjects. Results Nineteen subjects were recruited and completed the protocol. The subject pool was primarily white, female, 80+ and in possession of above average education. There was a significant relationship between MMSE® scores and the percentage of task success (z = −2.03, p = 0.04). Subjects with MMSE® scores of 24+ (no cognitive impairment) successfully completed an average of 69.0% of tasks vs. the 34.7% performance for those in the cognitively impaired group (<24). Six of the unimpaired group also succeeded at meeting the 85% (6 out of 7 correct) threshold, while none of the impaired group was able to. No subject with cognitive impairments (<24 MMSE®) completed more than 5/7 (71.4%) of their tasks. Two of the impaired subjects failed all of the tasks. Advising medical students is a challenging task. Faculty who serve as advisors for students require specific skills and knowledge to do their jobs effectively. Career choice is one of the many complex issues about which medical students often seek assistance from a faculty advisor. The authors present a case of a third-year medical student with career indecision, with a focus on the various factors that may be influencing her thinking about career choice. Key advising principles are provided as a framework for the discussion of the case and include reflection, self-disclosure, active listening, support and advocacy, confidentiality, and problem solving. These principles were developed as part of the Advising Case Conference series of the Johns Hopkins University School of Medicine Colleges Advisory Program. Emergent themes from the case included a student's evolving professional identity, a student's distress and burnout, lifestyle considerations, and advisor bias and self-awareness. The authors propose reflective questions to enhance meaningful discussions between the advisor and student and assist in problem solving. Many of these questions, together with the key advising principles, are generalizable to a variety of advising scenarios between advisors and learners at all levels of training. PMID:20354382

This paper describes an agent that shows advices for supporting mediator on our online mediation support system. The purpose of the advice is an education of mediator, and the agent presents it instead of the teacher. In this research, at first, we defined a mediation model that is an argumentation model between 3 people. Then, we defined some advice models based on the mediation model. The advice models create advice elements. The adviser agent monitors the mediation, gathers advice elements referring to advice models, and creates advice from elements according to the mediation scene. As a result, it becomes possible that is advising instead of the teacher according to the situation, education purpose and learner's level. The relationship between American presidents and science has always been surprisingly idiosyncratic. Chief executives from Franklin Roosevelt to Ronald Reagan have adopted very different approaches to dealing with the scientific community. The early history of presidential science advising is the story of a few energetic individuals who sought to influence government policy when there was no established method for scientists to approach the White House. President Eisenhower created such a channel in 1957 with the President's Science Advisory Committee [PSAC]. Under Ike, advising the president became for the first time a systematic and formal enterprise for scientists. But the decades after Sputnik witnesses a steady waning of the importance of the president's scientists. This decline was accelerated by the Vietnam war and controversies over anti-ballistic missile defense and the supersonic transport, culminating in the abolition of PSAC by the Nixon administration in 1973. To some observers, science advising reached its nadir a decade later, with President Reagan's announcement of the Strategic Defense Initiative (SDI). The advent of "Star Wars" demonstrated the influence of a small, select group of politically-active scientists: a consultative process reminscient of the ad hoc situation pre-PSAC.

During the period of 25-28 April 2011, the southeastern United States was raked by several episodes of severe weather. Numerous tornadoes caused extensive damage and, tragically, the deaths of more than 300 people. Meteorologists from the National Oceanic and Atmospheric Administration's (NOAA) National Weather Service (NWS) performed extensive surveys to assess the intensity, duration, and ground track of tornadoes reported during the event. Use of digitized aerial photographs for flood damage assessment in agriculture is new and largely untested. However, under flooding circumstances similar to the 1975 Red River Valley flood, computer-assisted techniques can be extremely useful, especially if detailed crop damage estimates are needed within a relatively short period of time. Airphoto interpretation techniques, manual or computer-assisted, are not intended to replace conventional ground survey and sampling procedures. But their use should be considered a valuable addition to the tools currently available for assessing agricultural flood damage.

Objectives To understand views of pharmacy advisers about smoker recruitment and retention in the National Health Service community pharmacy stop smoking programme. Design Thematic framework analysis of semistructured, in-depth interviews applying the Theoretical Domains Framework and COM-B behaviour change model. We aimed to identify aspects of adviser behaviour that might be modified to increase numbers joining and completing the programme. Participants 25 stop smoking advisers (13 pharmacists and 12 support staff). Setting 29 community pharmacies in 3 inner east London boroughs. Results Advisers had preconceived ideas about smokers likely to join or drop out and made judgements about smokers' readiness to quit. Actively recruiting smokers was accorded low priority due in part to perceived insufficient remuneration to the pharmacy and anticipated challenging interactions with smokers. Suggestions to improve smoker recruitment and retention included developing a more holistic and supportive approach using patient-centred communication. Training counter assistants were seen to be important as was flexibility to extend the programme duration to fit better with smokers’ needs. Conclusions Cessation advisers feel they lack the interpersonal skills necessary to engage well with smokers and help them to quit. Addressing advisers' behaviours about active engagement and follow-up of clients, together with regular skills training including staff not formally trained as cessation advisers, could potentially boost numbers recruited and retained in the stop smoking programme. Adjustments to the pharmacy remuneration structure to incentivise recruitment and to allow personalisation of the programme for individual smokers should also be considered. PMID:27388355

A simulation-based risk assessment approach is presented and is applied to the analysis of abort during the ascent phase of a space exploration mission. The approach utilizes groupings of launch vehicle failures, referred to as failure bins, which are mapped to corresponding failure environments. Physical models are used to characterize the failure environments in terms of the risk due to blast overpressure, resulting debris field, and the thermal radiation due to a fireball. The resulting risk to the crew is dynamically modeled by combining the likelihood of each failure, the severity of the failure environments as a function of initiator and time of the failure, the robustness of the crew module, and the warning time available due to early detection. The approach is shown to support the launch vehicle design process by characterizing the risk drivers and identifying regions where failure detection would significantly reduce the risk to the crew.

A key step in assisted reproduction is the assessment of embryo viability in order to identify the embryo(s) most likely to result in pregnancy. Currently used embryo assessment systems are largely based on morphology and cleavage rate. While these systems have been pivotal in improving implantation and pregnancy rates and reducing multiple gestations, their precision is still insufficient. The limitations of strategies based on morphology have led to the investigation of adjunctive technologies for non-invasive assessment of embryo viability in assisted reproduction. These include the measurement of glucose, pyruvate, or amino acid levels in the embryo culture media, assessment of oxygen consumption by the embryo, genomic and proteomic profiling, and most recently, analytical examination of the embryonic metabolome. As the number of ART cycles increases worldwide, improvements in the ability to quickly and non-invasively identify the best embryos for transfer become an increasingly more important goal for reproductive medicine. Conversion rates during video-assisted thoracoscopic lobectomy are reported, but no previous publications have classified the cause of conversion. The aim of the study was to develop a quality assessment tool [vascular, anatomy, lymph node, technical (VALT) 'Open'] to evaluate reasons and nature of conversion during the development of a video-assisted thoracoscopic lobectomy program. Between 2006 and 2008, 237 patients with a median age of 65 years underwent video-assisted thoracoscopic lobectomy primarily for lung. The number of video-assisted thoracoscopic lobectomy cases over open cases has increased over the period. Conversion rate has dropped from 15% (2006) to 11% (2008). A total of 32 cases required conversion. The VALT 'Open' classification for reason to convert and nature of conversion was used. The average length of stay was shorter for non-converted cases. No uncontrolled conversions where the patient was unstable were required, and in the 14 cases converted following some difficulty, such as pulmonary artery injury. A pattern to the learning curve became predictable. The quality assessment tool used (VALT 'Open') will allow cause of conversion and nature of conversion to be tracked and audited during the development of a video-assisted thoracoscopic surgery lobectomy program. Context The user interface development of assistive robotic manipulators can be traced back to the 1960s. Studies include kinematic designs, cost-efficiency, user experience involvements, and performance evaluation. This paper is to review studies conducted with clinical trials using activities of daily living (ADLs) tasks to evaluate performance categorized using the International Classification of Functioning, Disability, and Health (ICF) frameworks, in order to give the scope of current research and provide suggestions for future studies. Methods We conducted a literature search of assistive robotic manipulators from 1970 to 2012 in PubMed, Google Scholar, and University of Pittsburgh Library System – PITTCat. Results Twenty relevant studies were identified. Conclusion Studies were separated into two broad categories: user task preferences and user-interface performance measurements of commercialized and developing assistive robotic manipulators. The outcome measures and ICF codes associated with the performance evaluations are reported. Suggestions for the future studies include (1) standardized ADL tasks for the quantitative and qualitative evaluation of task efficiency and performance to build comparable measures between research groups, (2) studies relevant to the tasks from user priority lists and ICF codes, and (3) appropriate clinical functional assessment tests with consideration of constraints in assistive robotic manipulator user interfaces. In addition, these outcome measures will help physicians and therapists build standardized tools while prescribing and assessing assistive robotic manipulators. PMID:23820143

New advanced imaging systems will soon be capable of collecting enormous volumes of imagery, placing a significant burden on the imagery analysts (IAs) that exploit these data. ATRs and other image understanding tools offer a way to assist IAs in exploiting large volumes of imagery more effectively and efficiently. The Defense Advanced Research Project Agency (DARPA) Semi-Automated IMINT Processing (SAIP) Program focuses on these technologies to assist IAs in the timely exploitation of SAR imagery. The SAIP system is an integrated set of imagery exploitation tools designed to improve the capability of the IA to support military missions in a tactical environment. To assess the utility of the SAIP technology, a mix of live and playback exercises were conducted. IAs exploited the imagery with the assistance of the SAIP technology. As a benchmark for comparison, the same imagery was exploited in an operational exploitation system without the benefit of SAIP assistance. This paper presents the methodology for assessing exploitation performance and discusses issues related to scoring exploitation performance. The results of a recent assessment event illustrate the issues and provide guidance for future work in this area.

Interpretation of human genomes is a major challenge. We present the Scripps Genome ADVISER (SG-ADVISER) suite, which aims to fill the gap between data generation and genome interpretation by performing holistic, in-depth, annotations and functional predictions on all variant types and effects. The SG-ADVISER suite includes a de-identification tool, a variant annotation web-server, and a user interface for inheritance and annotation-based filtration. SG-ADVISER allows users with no bioinformatics expertise to manipulate large volumes of variant data with ease – without the need to download large reference databases, install software, or use a command line interface. This article reports the use of an expert panel to perform content validation of an experimental assessment process for the safety of assistive technology (AT) adopted by farmers with disabilities. The validation process was conducted by a panel of six experts experienced in the subject matter, i.e., design, use, and assessment of AT for farmers with disabilities. The exercise included an evaluation session and two focus group sessions. The evaluation session consisted of using the assessment process under consideration by the panel to evaluate a set of nine ATs fabricated by a farmer on his farm site. The expert panel also participated in the focus group sessions conducted immediately before and after the evaluation session. The resulting data were analyzed using discursive analysis, and the results were incorporated into the final assessment process. The method and the results are presented with recommendations for the use of expert panels in research projects and validation of assessment tools. PMID:21919319

Robot-assisted rehabilitation has been developed and proved effective for motion function recovery. Humanization is one of the crucial issues in the designing of robot-based rehabilitation system. However, most of the previous investigations focus on the simplex position control when comes to the control system design of robot-assisted passive training, and pay little attention to the dynamic adjustment according to the patient's performances. This paper presents a novel method to design the passive training system using a developed assessing-and-regulating section to online assess the subject's performances. The motion regulating mechanism is designed to dynamically adjust the training range and motion speed according to the actual performances, which is helpful to improve the humanization of the rehabilitation training. Moreover, position-based impedance control is adopted to achieve compliant trajectory tracking movement. One of the most important signs of systemic disease that presents on the retina is vascular abnormalities such as in hypertensive retinopathy. Manual analysis of fundus images by human readers is qualitative and lacks in accuracy, consistency and repeatability. Present semi-automatic methods for vascular evaluation are reported to increase accuracy and reduce reader variability, but require extensive reader interaction; thus limiting the software-aided efficiency. Automation thus holds a twofold promise. First, decrease variability while increasing accuracy, and second, increasing the efficiency. In this paper we propose fully automated software as a second reader system for comprehensive assessment of retinal vasculature; which aids the readers in the quantitative characterization of vessel abnormalities in fundus images. This system provides the reader with objective measures of vascular morphology such as tortuosity, branching angles, as well as highlights of areas with abnormalities such as artery-venous nicking, copper and silver wiring, and retinal emboli; in order for the reader to make a final screening decision. To test the efficacy of our system, we evaluated the change in performance of a newly certified retinal reader when grading a set of 40 color fundus images with and without the assistance of the software. The results demonstrated an improvement in reader's performance with the software assistance, in terms of accuracy of detection of vessel abnormalities, determination of retinopathy, and reading time. This system enables the reader in making computer-assisted vasculature assessment with high accuracy and consistency, at a reduced reading time. PMID:25571442

Metacognitive test taking strategies have proven advantageous in improving content-based test scores in a wide variety of disciplines and age/grade levels using traditional paper-and-pencil tests. However, despite the increase in computer assisted assessment (CAA), little research has examined whether these test taking strategies are effective for computer assisted tests. Research was conducted to determine if learning a proven test taking strategy would improve the online quiz scores of six university students in an introductory chemistry course intended for science, technology, engineering and math majors. Participants completed six to ten chemistry quizzes prior to intervention---learning the test taking strategy---and four to eight chemistry quizzes after intervention. Results indicated that, while students learned the strategy, it had little effect on their online chemistry quiz scores. Additionally, at the end of the semester, participants completed a satisfaction survey indicating general satisfaction with having learned the test taking strategy and generalization to other courses and types of tests. Furthermore, results suggest that adaptations to the on-line delivery method of the quizzes and to the test taking strategies may improve the robustness of the effect. Due to the increased use of computer assisted assessment, additional research is warranted to determine appropriate test taking strategies for online tests.

The ArmAssist is a low-cost robotic system for post-stroke upper-limb telerehabilitation based on serious games. The system incorporates a set of games for the assessment of arm function, the ArmAssist Assessment (AAA), which allows a remote monitoring of the progress of the patient and an automatic adaptation of the therapy. In this study, different components of the AAA are compared against three widely-used clinical tests, the Fugl-Meyer Assessment (FMA) , the Action Research Arm Test (ARAT)  and the Wolf Motor Function Test (WMFT)  in order to select the most clinically meaningful ones for the final score provided to patients and therapist, and evaluate their capability to predict or even improve some aspects of these standard scales. All four tests were performed in 38 separate sessions in 19 post-stroke individuals in their sub-acute phase, as part of a broader study. Statistically significant correlation could be shown with the three clinical tests. These preliminary results are promising for the validation of AAA as a fast, automatic and clinically meaningful tool for remote progress assessment and therapy adaptation; however, more data and further analysis is needed to confirm this. The purpose of this project was to assess the relationships between two federal programs that support low income households, the Weatherization Assistance Program (WAP) and the Low Income Home Energy Assistance Program (LIHEAP). The specific question addressed by this research is: what impact does weatherizing homes of LIHEAP recipients have on the level of need for LIHEAP assistance? The a priori expectation is that the level of need will decrease. If this is the case, then it can be argued that a non-energy benefit of WAP is the reduction in the level of need for LIHEAP assistance for households receiving weatherization assistance. The study area for this project was Boston, Massachusetts, which is representative of large northern urban areas. Additionally, Boston was chosen because one of its social service agencies, Action for Boston Community Development (ABCD), administers both WAP and LIHEAP programs. ABCD has a substantial client base of low-income households and was willing to cooperate in this study. In the State of Massachusetts, an income test is used to determine whether low-income households qualify for standard LIHEAP benefits. Benefits provided to eligible households are determined by a schedule that gauges benefit levels based on household income and number of members in the household. Additionally, households that consume large amounts of primary heating fuel can also qualify an additional high energy subsidy. It was expected that weatherization's biggest influence on the LIHEAP program would be in reducing the number of households qualifying for high energy subsidies. Data were collected for three groups of households that received both weatherization and LIHEAP assistance and for one control group that only received LIHEAP assistance. This article describes and illustrates a framework that may be used when assessing the moral basis of everyday issues that can affect residents of assisted living facilities. The "taxonomy of everyday ethical issues" was an outcome of ethnographic research focused on care of nursing home residents with dementia. However, in this article it is shown to be equally relevant for conceptualizing issues affecting cognitively intact as well as cognitively impaired residents across these two settings. The taxonomy is grounded in some of the more common cultural characteristics of congregate living arrangements for elderly individuals who have needs for varying levels of compensatory and therapeutic oversight by a staff of service providers. Almost every action of modern government has some scientific and technical component. However, most senior officials who must set policy and make decisions have little or no scientific training. As a result a small, but growing, number of professional scientists have left their research careers for new ones providing the needed technical advice. Interestingly enough, the job of "science adviser" is very different in the Executive Branch than it is in Congress. The major part of that difference comes from the responsibilities of the parent organization: the Executive actually sets the policies, proposes budgets, and then must perform. As science adviser to the Arms Control and Disarmament Agency and, after its merger with the State Department, I felt that I had a direct effect on how some issues were resolved. Congress, on the other hand, has the responsibility for authorizing and appropriating funds and setting the terms for their use. It exerts much of its power through holding hearings to make points to the public and the administration, but the adviser is usually placed bureaucratically much closer to the Senator or Congressman being advised than to a principal within the Executive Branch and may have more opportunities to communicate with his boss A science adviser is paid to advise on science, not policy, and must do his or her best not to shape the science to fit a desired outcome, the adviser's or the boss's. There are never enough scientists on staff to cover the territory; in all likelihood, there never will be. That makes it incumbent upon the adviser to reach out to his colleagues in ever-widening circles and across boundaries of disciplines. Irradiation-assisted stress corrosion cracking is a key materials degradation issue in today s nuclear power reactor fleet and affects critical structural components within the reactor core. The effects of increased exposure to irradiation, stress, and/or coolant can substantially increase susceptibility to stress-corrosion cracking of austenitic steels in high-temperature water environments. . Despite 30 years of experience, the underlying mechanisms of IASCC are unknown. Extended service conditions will increase the exposure to irradiation, stress, and corrosive environment for all core internal components. The objective of this effort within the Light Water Reactor Sustainability program is to evaluate the response and mechanisms of IASCC in austenitic stainless steels with single variable experiments. A series of high-value irradiated specimens has been acquired from the past international research programs, providing a valuable opportunity to examine the mechanisms of IASCC. This batch of irradiated specimens has been received and inventoried. In addition, visual examination and sample cleaning has been completed. Microhardness testing has been performed on these specimens. All samples show evidence of hardening, as expected, although the degree of hardening has saturated and no trend with dose is observed. Further, the change in hardening can be converted to changes in mechanical properties. The calculated yield stress is consistent with previous data from light water reactor conditions. In addition, some evidence of changes in deformation mode was identified via examination of the microhardness indents. This analysis may provide further insights into the deformation mode under larger scale tests. Finally, swelling analysis was performed using immersion density methods. Most alloys showed some evidence of swelling, consistent with the expected trends for this class of alloy. The Hf-doped alloy showed densification rather than swelling. This observation may be

Federal legislation and DOE Orders require sites to develop waste minimization/pollution prevention programs. In response to these requirements, the Kansas City Plant developed a pollution prevention tool called a pollution prevention opportunity assessment (PPOA). Pilot assessments resulted in the development of a graded approach to reduce the amount of effort required for activities that utilized nonhazardous and/or low-volume waste streams. The project`s objectives in FY95 were to validate DOE`s PPOA Graded Approach methodology, provide PPOA training and technical assistance to interested DOE personnel and DOE contractors, enhance the methodology with energy analysis and tools for environmental restoration activities, implement a DOE-wide PPOA database, and provide support to DOE EM-334 in the completion of a report which estimates the future potential for pollution prevention and waste minimization in the DOE complex.

We used concept mapping for the assessment, applying a pre-test/post-test design. In our study, 106 9th graders cooperated by working in pairs ( n = 53) for six lessons. As an interdisciplinary learning activity in such complex knowledge domains has to combine many different aspects, we focused on long-term knowledge. Learners working cooperatively in dyads constructed computer-supported concept maps which were analysed by specific software. The data analysis encompassed structural aspects of the knowledge corresponding to a target reference map. After the learning unit, the results showed the acquisition of higher-order domain-specific knowledge structures which indicates successful interdisciplinary learning through the hypermedia learning environment. The benefit of using a computer-assisted concept mapping assessment for research in science education, and in science classrooms is considered.

Since July 1992, the WHO Regional Office for Europe has been using epidemiological and public health assessment techniques to guide its interventions in the crisis in the former Yugoslavia. WHO field operations have evolved into the largest emergency relief operation ever undertaken in the European Region. The WHO programme of humanitarian assistance focuses on five major areas: public health, equipment, supplies and logistic support, support to war victims, primary health care for refugees and rehabilitation of the health care system. The main thrust of the programme is to implement the public health measures required for survival. The first step is to provide health intelligence for international humanitarian assistance programmes through health and nutrition monitoring. This leads to targeted intervention. While involved in all aspects of relief, WHO clearly justifies its presence by its technical focus, bringing its network of expertise and experience to bear on the assessment of need and the coordination of intervention. Three examples of this approach are given: the nutrition programme, which includes emergency food aid and health and nutrition monitoring, the winter protection programme in Bosnia and Herzegovina, and the medical kit programme, involving the design and use of special kits to cover basic emergency needs for medical supplies to the maximum number of people. Robotic assistance is increasingly used in neurological rehabilitation for enhanced training. Furthermore, therapy robots have the potential for accurate assessment of motor function in order to diagnose the patient status, to measure therapy progress or to feedback the movement performance to the patient and therapist in real time. We investigated whether a set of robot-based assessments that encompasses kinematic, kinetic and timing metrics is applicable, safe, reliable and comparable to clinical metrics for measurement of arm motor function. Twenty-four healthy subjects and five patients after spinal cord injury underwent robot-based assessments using the exoskeleton robot ARMin. Five different tasks were performed with aid of a visual display. Ten kinematic, kinetic and timing assessment parameters were extracted on joint- and end-effector level (active and passive range of motion, cubic reaching volume, movement time, distance-path ratio, precision, smoothness, reaction time, joint torques and joint stiffness). For cubic volume, joint torques and the range of motion for most joints, good inter- and intra-rater reliability were found whereas precision, movement time, distance-path ratio and smoothness showed weak to moderate reliability. A comparison with clinical scores revealed good correlations between robot-based joint torques and the Manual Muscle Test. Reaction time and distance-path ratio showed good correlation with the “Graded and Redefined Assessment of Strength, Sensibility and Prehension” (GRASSP) and the Van Lieshout Test (VLT) for movements towards a predefined position in the center of the frontal plane. In conclusion, the therapy robot ARMin provides a comprehensive set of assessments that are applicable and safe. The first results with spinal cord injured patients and healthy subjects suggest that the measurements are widely reliable and comparable to clinical scales for arm motor function. : Studies of stroke patients undergoing robot-assisted rehabilitation have revealed various kinematic parameters describing movement quality of the upper limb. However, due to the different level of stroke impairment and different assessment criteria and interventions, the evaluation of the effectiveness of rehabilitation program is undermined. This paper presents a systematic review of kinematic assessments of movement quality of the upper limb and identifies the suitable parameters describing impairments in stroke patients. A total of 41 different clinical and pilot studies on different phases of stroke recovery utilizing kinematic parameters are evaluated. Kinematic parameters describing movement accuracy are mostly reported for chronic patients with statistically significant outcomes and correlate strongly with clinical assessments. Meanwhile, parameters describing feed-forward sensorimotor control are the most frequently reported in studies on sub-acute patients with significant outcomes albeit without correlation to any clinical assessments. However, lack of measures in coordinated movement and proximal component of upper limb enunciate the difficulties to distinguish the exploitation of joint redundancies exhibited by stroke patients in completing the movement. To measure the early impact of robot-assisted partial nephrectomy (RAPN) on renal function as assessed by renal scan (Tc 99m-DTPA), addressing the issue of risk factors for ischemic damage to the kidney. All patients undergoing RAPN for cT1 renal masses between June 2013 and May 2014 were included in this prospective study. Renal function as expressed by glomerular filtration rate (GFR) was assessed by Technetium 99m-diethylenetriaminepentaacetic acid (Tc 99m-DTPA) renal scan preoperatively and postoperatively at 1 month in every patient. A multivariable analysis was used for the determination of independent factors predictive of GFR decrease of the operated kidney. Overall, 32 patients underwent RAPN in the time interval. Median tumor size, blood loss, and ischemia time were 4 cm, 200 mL, and 24 min, respectively. Two grade III complications occurred (postoperative bleeding in the renal fossa, urinoma). The GFR of the operated kidney decreased significantly from 51.7 ± 15.1 mL/min per 1.73 m(2) preoperatively to 40, 12 ± 12.4 mL/min per 1.73 m(2) 1 month postoperatively (p = 0.001) with a decrease of 22.4 %. On multivariable analysis, only tumor size (p = 0.05) was a predictor of GFR decrease of the operated kidney. Robotic-assisted partial nephrectomy had a detectable impact on early renal function in a series of relatively large tumors and prevailing intermediate nephrometric risk. A mean decrease of 22 % of GFR as assessed by renal scan in the operated kidney was found at 1 month postoperatively. In multivariable analysis, tumor size only was a significant predictor of renal function loss. PMID:26994776

We present a prototype human assistant system for space crew assessment and mission management. Our system is based on case episodes from American and Russian space missions and analog environments such as polar stations and undersea habitats. The general domain of small groups in isolated and confined environments represents a near ideal application area for case-based reasoning (CBR) - there are few reliable rules to follow, and most domain knowledge is in the form of cases. We define the problem domain and outline a unique knowledge representation system driven by conflict and communication triggers. The prototype system is able to represent, index, and retrieve case studies of human performance. We index by social, behavioral, and environmental factors. We present the problem domain, our current implementation, our research approach for an operational system, and prototype performance and results.
One of the tools used in a successful pollution prevention program is a pollution prevention opportunity assessment (PPOA). The purpose of this project was to share the Kansas City Plant`s (KCP`s) PPOA expertise with other Department of Energy (DOE) personnel and DOE contractors. This consisted of five major activities in FY94: (1) Validating DOE`s PPOA Graded Approach methodology; (2) Providing DOE-funded PPOA training and technical assistance to interested DOE personnel and DOE contractors; (3) Developing a promotional campaign; (4) Conducting a feasibility study to develop a DOE-wide PPOA information exchange mechanism; and (5) Organizing and coordinating information sharing among related DOE EM-334 pollution prevention projects. This report documents the FY94 efforts, lessons learned, and future plans for all of the PPOA-related activities.

In an era when health resources are increasingly constrained, international organisations are transitioning from directly managing health services to providing technical assistance (TA) to in-country owners of public health programmes. We define TA as: A dynamic, capacity-building process for designing or improving the quality, effectiveness, and efficiency of specific programmes, research, services, products, or systems’. TA can build sustainable capacities, strengthen health systems and support country ownership. However, our assessment of published evaluations found limited evidence for its effectiveness. We summarise socio-behavioural theories relevant to TA, review published evaluations and describe skills required for TA providers. We explore challenges to providing TA including cost effectiveness, knowledge management and sustaining TA systems. Lastly, we outline recommendations for structuring global TA systems. Considering its important role in global health, more rigorous evaluations of TA efforts should be given high priority. PMID:22606939

Background: Peer-assisted learning (PAL) is the development of knowledge and skill through active help and support of equals. However, this has not been tested in medical education in India. Objectives: To assess the effectiveness of PAL on improvement in cognitive assessment scores and its acceptance among undergraduate medical students in one public teaching medical university in North India. Methodology: After approval from the Institutional Ethics Committee, three PAL sessions, 1 per week, each on specific topic, were conducted using small group discussion methodology with a faculty contact and student leader and 4–6 peer-learners, in 9th semester MBBS students. A pretest with multiple choice questions (MCQs) was followed by distribution of learning objectives and list of resource material. PAL session was conducted after 72 h, followed by posttest by MCQs and then focus group discussion (FGD) on students’ experiences. Results: Of the 26 students enrolled, three PAL sessions was completed by 22 (84.6%) students. The correlation coefficient between pre- and post-test scores was 0.48 (P < 0.0001), with a 24.2% improvement in posttest scores. In the nine FGDs most said that PALs helped in the better preparation of the topic, clarifying doubts, lessened examination anxiety, improved communication skills, and increased self-confidence. Conclusion: PAL was well accepted, and it improved assessment scores. Therefore, it can be adopted for teaching selected topics across all subjects of MBBS course. PMID:26380206

Due to current or predicted health workforce shortages, policy makers worldwide are addressing issues of task allocation, skill mix, and role substitution. This article presents an example of this process in the United States (US). Health workforce analysts recommend that US physician workforce planning account for the impact of physician assistants (PAs) and nurse practitioners (NPs). We examined 40 state workforce assessments in order to identify best practices for including PAs/NPs. Most assessments (about 60%) did not include PAs/NPs in provider counts, workforce projections or recommendations. Only 35% enumerated PAs/NPs. Best practices included use of an accurate data source, such as state licensing data, and combined workforce planning for PAs, NPs, and physicians. Our findings suggest that interprofessional medical workforce planning is not the norm among the states in the US. The best practices that we identify may be instructive to states as they develop methods for assessing workforce adequacy. Our discussion of potential barriers to interprofessional workforce planning may be useful to policy makers worldwide as they confront issues related to professional boundaries and interprofessional workforce planning. Background: Risk-of-bias assessments are now a standard component of systematic reviews. At present, reviewers need to manually identify relevant parts of research articles for a set of methodological elements that affect the risk of bias, in order to make a risk-of-bias judgement for each of these elements. We investigate the use of text mining methods to automate risk-of-bias assessments in systematic reviews. We aim to identify relevant sentences within the text of included articles, to rank articles by risk of bias and to reduce the number of risk-of-bias assessments that the reviewers need to perform by hand. Methods: We use supervised machine learning to train two types of models, for each of the three risk-of-bias properties of sequence generation, allocation concealment and blinding. The first model predicts whether a sentence in a research article contains relevant information. The second model predicts a risk-of-bias value for each research article. We use logistic regression, where each independent variable is the frequency of a word in a sentence or article, respectively. Results: We found that sentences can be successfully ranked by relevance with area under the receiver operating characteristic (ROC) curve (AUC) > 0.98. Articles can be ranked by risk of bias with AUC > 0.72. We estimate that more than 33% of articles can be assessed by just one reviewer, where two reviewers are normally required. Conclusions: We show that text mining can be used to assist risk-of-bias assessments. PMID:26659355

The purpose of this study was to examine how academic advising impacts minority STEM students' academic success and their level of satisfaction. The study also explored minority STEM students' perceptions of academic advising based on their experience. The sample included 188 sophomore and junior STEM students attending an HBCU in southeastern Louisiana. Participants in the study completed the Academic Advising Inventory (AAI). Some students also participated in a focus group or virtual interview. An independent t-test found no difference between the GPAs of STEM students who received developmental advising as opposed to prescriptive advising. A one-way ANOVA found no significant difference between STEM students' GPAs based on the frequency and duration of their advising sessions. A Mann-Whitney U test determined that STEM students who were prescriptively advised were significantly more satisfied with advising than STEM students who were developmentally advised. A Mann-Whitney U also determined that STEM students who were satisfied with their education were significantly more dissatisfied with academic advising than STEM students who were dissatisfied with their education. A Kruskal-Wallis H test determined there was no significant difference between STEM students' satisfaction with advising and the frequency of their advising sessions. A Kruskal-Wallis H also determined that STEM students who spent less than 15 minutes or more than 1 hour were the most satisfied with advisement. The majority of STEM students perceived academic advising had little impact on their GPA. However, STEM students perceived academic advising as having an impact on their satisfaction with the university. The majority of STEM students perceived academic advising as useful.

The dialog among investigators in academia, industry, NIH, and the FDA has grown in recent years on topics of historic interest to attendees of these SPIE sub-conferences on Image Perception, Observer Performance, and Technology Assessment. Several of the most visible issues in this regard have been the emergence of digital mammography and modalities for computer-assisted detection and diagnosis in breast and lung imaging. These issues appear to be only the "tip of the iceberg" foreshadowing a number of emerging advances in imaging technology. So it is timely to make some general remarks looking back and looking ahead at the landscape (or seascape). The advances have been facilitated and documented in several forums. The major role of the SPIE Medical Imaging Conferences i well-known to all of us. Many of us were also present at the Medical Image Perception Society and co-sponsored by CDRH and NCI in September of 2001 at Airlie House, VA. The workshops and discussions held at that conference addressed some critical contemporary issues related to how society - and in particular industry and FDA - approach the general assessment problem. A great deal of inspiration for these discussions was also drawn from several workshops in recent years sponsored by the Biomedical Imaging Program of the National Cancer Institute on these issues, in particular the problem of "The Moving Target" of imaging technology. Another critical phenomenon deserving our attention is the fact that the Fourth National Forum on Biomedical Imaging in Oncology was recently held in Bethesda, MD., February 6-7, 2003. These forums are presented by the National Cancer Institute (NCI), the Food and Drug Administration (FDA), the Centers for Medicare and Medicaid Services (CMS), and the National Electrical Manufacturers Association (NEMA). In the last 2 decades major advances have been made in the field of assessment methods for medical imaging and computer-assist systems through the use of the paradigm of the receiver operating characteristic (ROC) curve. In the most recent decade this methodology was extended to embrace the complication of reader variability through advances in the multiple-reader, multiple-case (MRMC) ROC measurement and analysis paradigm. Although this approach has been widely adopted by the imaging research community, some investigators appear averse to it, possibly from concern that it could place a greater burden on the scarce resources of patient cases and readers compared to the requirements of alternative methods. The present communication argues, however, that the MRMC ROC approach to assessment in the context of reader variability may be the most resource-efficient approach available. Moreover, alternative approaches may also be statistically uninterpretable with regard to estimated summary measures of performance and their uncertainties. The authors propose that the MRMC ROC approach be considered even more widely by the larger community with responsibilities for the introduction and dissemination of medical imaging technologies to society. The Nasal Obstruction Symptom Evaluation (NOSE) scale is a reliable and valid instrument used widely in otorhinolaryngology to evaluate nasal obstruction symptoms in patients with nasal disorders. The purpose of this study was to assess nasal obstruction symptoms prospectively in patients undergoing surgically assisted rapid maxillary expansion (SARME) using the NOSE scale. Sixteen patients were studied (mean age 31±7.7 years), 10 women and six men, all with a transverse maxillary deficiency and an indication for SARME. Hyrax type devices were placed preoperatively and SARME was performed using Kraut's technique. The NOSE scale was applied prospectively to assess nasal obstruction symptoms. The results were recorded for each score on a scale ranging from 0 to 4, and these scores were multiplied by 5, generating a balanced scale from 0 to 100. Data were stratified according to NOSE scores, and nasal obstruction was categorized as mild (0-25), moderate (26-50), or severe (>50). The questionnaire was administered twice, first preoperatively and then at 6 months after surgery, and the results compared. Data were analyzed statistically using SAS statistical package software and showed that patients experienced a subjective improvement or did not have a worsening of nasal obstruction symptoms after SARME. Objective. To describe the implementation and perceptions of an advising program’s meet-and-greet session on student/faculty interactions. Design. Student advisees and faculty advisors attended a meet-and-greet program designed to facilitate introductions. Two online surveys evaluating program perceptions were electronically distributed to participants. Assessment. Twenty-eight advisors and 226 students attended; 17 faculty members and 42% (n=95) of students completed the survey. Advisors and advisees found the program valuable (100%, 85%) and recommended holding it again (100%, 93%), respectively. Most advisors agreed that the event improved success in meeting advisees while reducing time needed to schedule and meet with advisees. Students felt more comfortable contacting advisors after participating, with 83% agreeing it was more convenient than scheduling separate meeting times. Conclusion. An advising meet-and-greet program facilitated initial advisee/advisor meetings while reducing self-reported faculty time/resources. This activity could be implemented by other institutions seeking to promote student advising relationships. PMID:26889062

This in vitro study compares the hemodynamic performance of the Norwood and the Glenn circulations to assess the performance of a novel assisted bidirectional Glenn (ABG) procedure for stage one single ventricle surgery. In the ABG, the flow in a bidirectional Glenn procedure is assisted by injection of a high-energy flow stream from the systemic circulation using an aorta-caval shunt with nozzle. The aim is to explore experimentally the potential of the ABG as a surgical alternative to current surgical practice. The experiments are directly compared against previously published numerical simulations. A multiscale mock circulatory system was used to measure the hemodynamic performance of the three circulations. For each circulation, the system was tested using both low and high values of pulmonary vascular resistance. Resulting parameters measured were: pressure and flow rate at left/right pulmonary artery and superior vena cava (SVC). Systemic oxygen delivery (OD) was calculated. A parametric study of the ratio of ABG nozzle to shunt diameter was done. We report time-based comparisons with numerical simulations for the three surgical variants tested. The ABG circulation demonstrated an increase of 30-38% in pulmonary flow with a 2-3.7 mmHg increase in SVC pressure compared to the Glenn and a 4-14% higher systemic OD than either the Norwood or the Glenn. The nozzle/shunt diameter ratio affected the local hemodynamics. These experimental results agreed with those of the numerical model: mean flow values were not significantly different (p > 0.05) while mean pressures were comparable within 1.2 mmHg. The results verify the approaches providing two tools to study this complicated circulation. Using a realistic experimental model we demonstrate the performance of a novel surgical procedure with potential to improve patient hemodynamics in early palliation of the univentricular circulation.
