In this work is presented the development of a mechatronics device to simulate the interaction of the surgeon with the surgical instrument (resectoscope) used during a Transurethral Resection of the Prostate (TURP). Our mechatronics interface is part of a computer assisted system for training in TURP, which is based on a 3D graphics model of the prostate which can be deformed and resected interactively by the user. The mechatronics interface, is the device that the urology residents will manipulate to simulate the movements performed during surgery. Our current prototype has five degrees of freedom, which are enough to have a realistic simulation of the surgery movements. Two of these degrees of freedom are linear, to determinate the linear displacement of the resecting loop and the other three are rotational to determinate three directions and amounts of rotation.

The United States Army, Berlin and its Berlin Brigade have an urgent requirement for a training system that can provide a realistic, stress-filled, simulated combat training environment for an urban battlefield. The simulation system will support the training of division/brigade/battalion commanders and their staffs to exercise procedures and decision making which will be essential to win on a modern urban battlefield. The current manual simulation used by the Berlin Brigade requires an excessive amount of time for pre-exercise set up and training of players and controllers. The manual simulation requires a large training support staff consisting of 40 to 60 personnel per shift. In addition, the current simulation has numerous modeling deficiencies reducing the quality of the combat representation to the extent that realistic training is not feasible in many circumstances. The computer assisted simulation system UCCATS will provide a means to more efficiently support realistic training for combat in the urban environment. UCCATS will support the training of a division/brigade/battalion commander and his staff. It will exercise commanders and staff in the command and control of combined arms operations in an urban terrain environment.

Due to increasing requirements on the accuracy and reproducibility of measurement results together with a rapid development of novel technologies for the execution of measurements, there is a high demand for adequately qualified metrologists. Accordingly, a variety of training offers are provided by machine manufacturers, universities and other institutions. Yet, for an interested learner it is very difficult to define an optimal training schedule for his/her individual demands. Therefore, a computer-based assistance tool is developed to support a demand-responsive scheduling of training. Based on the difference between the actual and intended competence profile and under consideration of amending requirements, an optimally customized qualification concept is derived. For this, available training offers are categorized according to different dimensions: regarding contents of the course, but also intended target groups, focus of the imparted competences, implemented methods of learning and teaching, expected constraints for learning and necessary preknowledge. After completing a course, the achieved competences and the transferability of gathered knowledge are evaluated. Based on the results, recommendations for amending measures of learning are provided. Thus, a customized qualification for manufacturing metrology is facilitated, adapted to the specific needs and constraints of each individual learner.

This study investigates three questions related to the problem of medication nonadherence among elders. First, does recall failure play a significant role in nonadherence? Recent research suggests that it may not. Second, can the new portable bar code scanner technology be used to study nonadherence? Other forms of monitoring are obtrusive or inaccurate. Finally, can inexpensive computer assisted instructions (CAI) be used to teach mnemonic techniques specifically designed to improve medication schedule recall? Current research on memory training teaches nonspecific mnemonics and uses the expensive classroom approach. Results of the present study suggest that physically active and cognitively alert elders do have significant nonadherence (control group = 32.0%) problems related to forgetting and that CAI courseware can significantly reduce (medication recall training group = 10.0%) this form of nonadherence. Portable bar code technology proved easy to use by elderly patients and provided detailed information about the type of forgetting underlying nonadherence. Most significant recall failure was in the complete forgetting to take medication rather than delays in medicating or overmedicating. PMID:3049749

The Liverpool Epidemiology Programme, based in the Liverpool School of Tropical Medicine, has designed a series of computer-based modules for use in distance learning. The purpose of this paper is to highlight the role of computers in training health workers in epidemiology in developing countries. The aim of the modules is to provide health workers with solutions to problems which they face in their everyday work. The modules are written in hypertext software for IBM compatible machines and interact with the epidemiological software Epi Info. Four modules are described: LEP-Nut which deals with nutritional surveillance, LEP-Ref which looks at the role of epidemiology within a refugee health care programme, LEP-Surv dealing with health surveillance and LEP-Rap which introduces the concept of rapid appraisal. Computer-assisted instruction (CAI) has been significantly advanced by the development of inexpensive multimedia personal computers (PCs). As a part of efforts to integrate PC workstations as the primary interface to the anatomic pathology information system, we undertook an evaluation and implementation of image-based resident workstations drawing on technology and software now available. The goal was to develop an integrated diagnostic and research data center using PC workstations. After considerable analysis we implemented a Resident's Resource Center (RRC) to augment the training environment of the resident pathologist by providing productivity tools for the writing, research, and presentation needs of the trainee. We also provided video- and text-based education applications specifically tailored to the training pathologist. This led to the creation of two types of Windows-based workstations. The Productivity Workstation consists of a flatbed scanner, laser printer, and photorecorder connected to a 486-type PC. The Education Workstation consists of a high-resolution monitor and video disc player with bar code scanner connected to a 486-type PC. We briefly review the literature concerning CAI in pathology; outline the hardware, software, personnel and cost concerns that we faced in setting up our RRC; provide a partial list of vendors and programs currently on the market; review the software we have installed; and discuss the results of our efforts. The rationale for using computers in psychotherapy includes the possibility that therapeutic software could improve the efficiency of treatment and provide access for greater numbers of patients. Computers have not been able to reliably duplicate the type of dialogue typically used in clinician-administered therapy. However, computers have significant strengths that can be used to advantage in designing treatment programs. Software developed for computer-assisted therapy generally has been well accepted by patients. Outcome studies have usually demonstrated treatment effectiveness for this form of therapy. Future development of computer tools may be influenced by changes in health care financing and rapid growth of new technologies. An integrated care delivery model incorporating the unique attributes of both clinicians and computers should be adopted for computer-assisted therapy. PMID:9292446

Upper extremity exercise training has been shown to improve clinical outcomes in different chronic health conditions. Arm-operated bicycles are frequently used to facilitate upper extremity training however effective use of these devices at patient homes is hampered by lack of remote connectivity with clinical rehabilitation team, inability to monitor exercise progress in real time using simple graphical representation, and absence of an alert system which would prevent exertion levels exceeding those approved by the clinical rehabilitation team. We developed an interactive biking exercise (iBikE) platform aimed at addressing these limitations. The platform uses a miniature wireless 3-axis accelerometer mounted on a patient wrist that transmits the cycling acceleration data to a laptop. The laptop screen presents an exercise dashboard to the patient in real time allowing easy graphical visualization of exercise progress and presentation of exercise parameters in relation to prescribed targets. The iBikE platform is programmed to alert the patient when exercise intensity exceeds the levels recommended by the patient care provider. The iBikE platform has been tested in 7 healthy volunteers (age range: 26-50 years) and shown to reliably reflect exercise progress and to generate alerts at pre-setup levels. Implementation of remote connectivity with patient rehabilitation team is warranted for future extension and evaluation efforts. PMID:23367319

The broad range of Computer Assisted Surgery (CAS) represents the integration of computer technology in surgical procedures for presurgical planning, guiding or manipulation. Surgical robots and surgical endoscopic navigation are the most challenging applications to urology. A surgical robot is defined as a computer-controlled manipulator with artificial sensing which can be programmed to move, and position tools to carry out surgical tasks. In urology, robots have been tested in two areas: endourology and laparoscopy. Surgical navigation allows the surgeon to process data from pre- and intraoperative sources, aiming at purification and presentation of the most relevant information. Image-guided systems (IGS), augmented reality (AR) and navigation in endoscopic soft tissue surgery represent the three main topics of surgical urological navigation. IGS involve matching the coordinates from medical imaging (preoperative registration) with coordinates from the patient in the operating room (registration and updating images). A successful programme is reported from Mozambique for training middle-level health workers to perform fairly advanced surgical procedures in remote areas where the services of consultants are virtually unobtainable. Manpower and financial constraints obliged Mozambique to train medical assistants to perform surgical work in rural areas, where three broad priorities were identified: pregnancy-related complications, trauma-related complications, and emergency inflammatory conditions. Since 1984, 20 health workers have emerged from three-year courses to become técnicos de cirurgía (assistant medical officers), and it is expected that there will be 46 by 1999. The training comprises two years of lectures and practical sessions in the Maputo Central Hospital, and a practical internship lasting a year at a provincial hospital. Three workshops organized since 1989 suggest that the upgraded personnel are performing well. More detailed evaluation and follow-up are in progress. Throughout 1995 a follow-up was conducted on 14 assistant medical officers. They performed 10,258 surgical operations, some 70% of which were emergency interventions. Low rates of complication occurred and postoperative mortality amounted to 0.4% and 0.1% in emergency and elective interventions respectively. PMID:10516791

Divided attention (DA), the ability to distribute cognitive resources among two or more simultaneous tasks, may be severely compromised after traumatic brain injury (TBI), resulting in problems with numerous activities involved with daily living. So far, no research has investigated whether the use of non-invasive brain stimulation associated with neuropsychological rehabilitation might contribute to the recovery of such cognitive function. The main purpose of this study was to assess the effectiveness of 10 transcranial direct current stimulation (tDCS) sessions combined with computer-assisted training; it also intended to explore the neural modifications induced by the treatment. Thirty-two patients with severe TBI participated in the study: 16 were part of the experimental group, and 16 part of the control group. The treatment included 20' of tDCS, administered twice a day for 5 days. The electrodes were placed on the dorso-lateral prefrontal cortex. Their location varied across patients and it depended on each participant's specific area of damage. The control group received sham tDCS. After each tDCS session, the patient received computer-assisted cognitive training on DA for 40'. The results showed that the experimental group significantly improved in DA performance between pre- and post-treatment, showing faster reaction times (RTs), and fewer omissions. No improvement was detected between the baseline assessment (i.e., 1 month before treatment) and the pre-training assessment, or within the control group. Functional magnetic resonance imaging (fMRI) data, obtained on the experimental group during a DA task, showed post-treatment lower cerebral activations in the right superior temporal gyrus (BA 42), right and left middle frontal gyrus (BA 6), right postcentral gyrus (BA 3) and left inferior frontal gyrus (BA 9). Computer-based training (CBT) is used in a new approach to training refinery operators. The system was developed at Mobil's U.S. headquarters, with field help. In the early 1980s the authors evaluated the mainframe Plato system from Control Data, considered by most the ''granddad'' of CBT systems. A couple of years later, when the CPM-based micro computers became popular, they traded in Plato for stand-alone desktop that offered most of the same features without the mainframe's cost and connectivity limitations. Their evaluation continued into the mid-1980s after IBM established its PC line which became the corporate hardware standard. CBT is both right for the times and particularly suited to the hydrocarbon processing industry's needs. Mobil has taken significant steps to benefit from all that CBT offers by developing two curricula for operator training. The first, the Fundamentals Series, provides the background needed for any operations assignment. The second curriculum, the Advanced Series, covers specific process unit job training.

In this paper we report our preliminary results of the development of a computer assisted system for breast biopsy. The system is based on tracked ultrasound images of the breast. A three dimensional ultrasound volume is constructed from a set of tracked B-scan images acquired with a calibrated probe. The system has been designed to assist a radiologist during breast biopsy, and also as a training system for radiology residents. A semiautomatic classification algorithm was implemented to assist the user with the annotation of the tumor on an ultrasound volume. Computer-assisted orthopaedic surgery (CAOS) is performed by digitizing the patient's anatomy, combining the images in a computerized system, and integrating the surgical instruments into the digitized image background. This allows the surgeon to navigate the surgical instruments and the bone in an improved, virtual visual environment. CAOS in traumatology is performed with images obtained by fluoroscopy, CT, or three-dimensional fluoroscopy. CAOS is used in basic trauma procedures for preoperative planning, fracture reduction, intramedullary nailing, percutaneous screw or plate fixation, and hardware or shrapnel removal. Potential benefits of CAOS include minimal invasiveness, increased accuracy, and decreased radiation exposure. Limitations include a significant learning curve, increased surgical time, requirements for special setup and equipment handling in the operating room, specialized technical support, and cost. Current evidence shows no advantage with CAOS in trauma cases compared with conventional methods. Prospective randomized trials and clinical outcomes are lacking. PMID:20435875

Computer-based instruction is being widely used in the education programs of many allied health professions. However, there has been little, if any, documentation of computer-based instruction use in athletic training education. The primary purpose of this study was to determine what percentage of undergraduate and graduate NATA-approved athletic training education programs are using some form of computer-based instruction (ie, computer-assisted instruction or interactive video). We also addressed the following research questions: 1) What athletic training educational software is currently being used by athletic training students and educators? 2) What factors currently impede the use of computer-based instruction in athletic training education? 3) What instructional methods are commonly used to incorporate computer-based instruction into the athletic training curricula? and 4) What are the attitudes of athletic training program directors toward the use of computer-based instruction in athletic training education? Surveys were mailed to the program directors (n = 97) of all graduate and undergraduate NATA-approved athletic training education programs. Eighty-six (87.7%) usable surveys were returned. Forty-eight (55.8%) of the respondents reported using some form of computer-based instruction in their athletic training education program; 47 (54.7%) used computer-assisted instruction and 9 (10.6%) used interactive video. Respondents also identified the educational software they use and their method for implementing this software. Software was used most often to supplement traditional instructional methods. A lack of funds was reported to be the primary impeding factor for those programs not using computer-based instruction. Respondents reported an overall positive attitude toward computer-based instruction use in athletic training education and indicated the need for increased development of athletic training/sports medicine software. In stroke survivors, motor impairment is frequently associated with degraded proprioceptive and/or somatosensory functions. Here we address the question of how to use robots to improve proprioception in these patients. We used an 'assist-as-needed' protocol, in which robot assistance was kept to a minimum and was continuously adjusted during exercise. To specifically train proprioceptive functions, we alternated blocks of trials with and without vision. A total of nine chronic stroke survivors participated in the study, which consisted of a total of ten 1-h exercise sessions. We used a linear mixed-effects statistical model to account for the effects of exercise, vision and the degree of assistance on the overall performance, and to capture both the systematic effects and the individual variations. Although there was not always a complete recovery of autonomous movements, all subjects exhibited an increased amount of voluntary control. Moreover, training with closed eyes appeared to be beneficial for patients with abnormal proprioception. Our results indicate that training by alternating vision and no-vision blocks may improve the ability to use proprioception as well as the ability to integrate it with vision. We suggest that the approach may be useful in the more general case of motor skill acquisition, in which enhancing proprioception may improve the ability to physically interact with the external world. It is used in urological surgeries as an assistance in diagnosis of lower urinary track diseases like Benign Prostatic Hyperplasia (BPH). Although there has been much research into intelligent tutoring systems (ITS), there are few authoring systems available that support ITS metaphors. Instructional developers are generally obliged to use tools designed for creating on-line books. We are currently developing an authoring environment derived from NASA's research on intelligent computer-aided training (ICAT). The ICAT metaphor, currently in use at NASA has proven effective in disciplines from satellite deployment to high school physics. This technique provides a personal trainer (PT) who instructs the student using a simulated work environment (SWE). The PT acts as a tutor, providing individualized instruction and assistance to each student. Teaching in an SWE allows the student to learn tasks by doing them, rather than by reading about them. This authoring environment will expedite ICAT development by providing a tool set that guides the trainer modeling process. Additionally, this environment provides a vehicle for distributing NASA's ICAT technology to the private sector.
This paper discusses a Computer-Aided-Instruction (CAI) course that has been implemented at Rocky Flats. Trying to train 2000 computer users and Computer Security Officers (CSO) has been a real dilemma at Rocky Flats. In order to resolve this problem, our Training Department implemented a CAI training course on Computer Security. Presently, we are training approximately 100 users/CSOs per month, and soon these numbers should increase. The duration of the User course is about an hour and the CSO's is about an hour and a half. CAI is being used on the unclassified VAX computer system in conjunction with IBM PC XTs.

Educators and learners need to explore these media and determine how they can best fit into a total learning experience. Computers should be used to enhance education and training, rather than replace the human instructor. The latest software and hardware are interesting to learners, but technology needs to be weighed along with outcomes of education. Over the next 10 years, many of the materials we use today for critical care education will be replaced with more advanced technologies. Optimal monitoring of vital parameters in critically ill patients should be performed continuously with non-invasive and automatic methods including trend presentation of the data. Continuous measurements are now in frequent use in intensive care units, and non-invasive techniques are being increasingly explored. Besides automatic registration, computerization of monitoring allows trend presentation of the measured values and thereby improves diagnostic and therapeutic procedures. In a national laboratory setting, it is necessary to provide radiation protection overview and training to diverse parts of the laboratory population. This includes employees at research reactors, accelerators, waste facilities, radiochemical isotope processing, and analytical laboratories, among others. In addition, our own radiation protection and monitoring staffs must be trained. To assist in the implementation of this full range of training, ORNL has purchased prepackaged computer-based training in health physics and technical mathematics with training modules that can be selected from many topics. By selection of specific modules, appropriate radiation protection review packages can be determined to meet many individual program needs. Because our radiation protection personnel must have some previous radiation protection experience or the equivalent of an associate's degree in radiation protection for entry level, the computer-based training will serve primarily as review of major principles. Others may need very specific prior training to make the computer-based training effective in their work situations. 4 refs.

With many surgical training programmes providing less time for training it can be challenging for trainees to acquire the necessary surgical skills to perform complex video assisted thoracoscopic surgery (VATS) lung resections. Indeed as the utilization of single port operations increases the need to approach the operating theatre with already-existing excellent hand-eye coordination skills increases. We suggest that there are a number of ways that trainees can begin to develop these necessary skills. Firstly, using computer games that involve changing horizons and orientations. Secondly, utilizing box-trainers to practice using the thoracoscopic instruments. Thirdly, learning how essential tools such as the stapler work. Trainees will then be able to progress to meaningfully assisting in theatre and indeed learning how to perform the operation themselves. At this stage is useful to observe expert surgeons whilst they operate—to watch both their technical and non-technical skills. Ultimately, surgery is a learned skill and requires implementation of these techniques over a sustained period of time. PMID:26697479
EXECUTIVE SUMMARY Objective The objective of this health technology policy assessment was to determine the effectiveness and cost-effectiveness of computer-assisted surgery with telemanipulators. The Technology The technology for computer-assisted surgery with telemanipulators is a robotic arm that carries an endoscope while two other manipulator arms carry interchangeable tools, such as scissors and grippers. In a master-slave telemanipulator system, the master may consist of a joystick input system, or for surgery, may mimic the motion of the slave robot, such as the da Vinci and ZEUS surgical systems. These systems are capable of telerobotic surgery, or surgery from remote locations. Review Strategy The Cochrane and INAHTA databases yielded 4 health technology assessments or systematic reviews on computer-assisted surgery using telemanipulators. A search of MEDLINE and EMBASE January 1, 2001 to November 24, 2003 was conducted. This search produced 448 studies, of which 19 met the inclusion criteria. Summary of Findings Published health technology assessments indicate that there are limited data from studies, although there is initial evidence of the safety and efficacy of telemanipulators in some procedures when they are used at large academic centres for surgery on selected patients. Most studies are Level 3 and 4 observational studies and assess a wide variety of surgical procedures. Limited studies indicate the promise of telemanipulators, but their efficacy is not fully established. In some procedures, the advantages that telemanipulators may offer may also be achieved by non-robotic minimally invasive/laparoscopic techniques. To date, cost-effectiveness has not been demonstrated. Patients who have undergone robotic surgery must be followed to further define outcomes (e.g., long-term quality of the graft after coronary arterial bypass graft [CABG] surgery). Due to anatomical variability and limited visibility of endoscopic image, endoscopic operations of nose and paranasal sinuses are ones of the most difficult surgical procedures. The field of operation often comprises anatomical structures, which often present anomalies. Computer-assisted navigational endoscopic surgery consists of routine tomography with the possibility of 3-axis projection allowing for localization of surgical instruments in proper relation to anatomic structures. This potential permits the surgeon to penetrate specific structures with surgical instruments and visualize their localization on computer tomography, which was earlier entered to the computer and projected. Projection of the images and endoscopic picture on the same monitor provides comfort to the operator and feeling of safety to the operated patient. The image analysis feature supplies a set of information necessary for safer and more effective procedure conduction and decreased number of complications. This technique may considerably contribute to training programs in endoscopic surgery. Computer-aided navigation in surgical procedures allows for precise biopsy specimen uptake for pathological examination, even in cases requiring precision up to 1 mm. The authors present an overview of surgical computer-aided navigation systems and their own experience in endoscopic ethmoid and maxillary sinus surgery performed with the use of computer-assisted navigation system.

Purpose: To create a more effective method of demonstrating complex subject matter in ophthalmology with the use of high end, 3-D, computer aided animation and interactive multimedia technologies. Specifically, to explore the possibilities of demonstrating the complex nature of the neuroophthalmological basics of the human oculomotor system in a clear and non confusing way, and to demonstrate new forms of retinal surgery in a manner that makes the procedures easier to understand for other retinal surgeons. Methods and Materials: Using Reflektions 4.3, Monzoom Pro 4.5, Cinema 4D XL 5.03, Cinema 4D XL 8 Studio Bundle, Mediator 4.0, Mediator Pro 5.03, Fujitsu-Siemens Pentium III and IV, Gericom Webgine laptop, M.G.I. Video Wave 1.0 and 5, Micrografix Picture Publisher 6.0 and 8, Amorphium 1.0, and Blobs for Windows, we created 3-D animations showing the origin, insertion, course, main direction of pull, and auxiliary direction of pull of the six extra-ocular eye muscles. We created 3-D animations that (a) show the intra-cranial path of the relevant oculomotor cranial nerves and which muscles are supplied by them, (b) show which muscles are active in each of the ten lines of sight, (c) demonstrate the various malfunctions of oculomotor systems, as well as (d) show the surgical techniques and the challenges in radial optic neurotomies and subretinal surgeries. Most of the 3-D animations were integrated in interactive multimedia teaching programs. Their effectiveness was compared to conventional teaching methods in a comparative study performed at the University of Vienna. We also performed a survey to examine the response of students being taught with the interactive programs. Surgical training systems based on virtual reality (VR) are highly desired as they offer a cost effective and efficient alternative compared to traditional training methods. Traditional surgical training methods require cadavers or plastic models which are costly. Cadavers cannot be used repeatedly and training with plastic models cannot provide the realistic experience. This paper describes a visualization to show the use of newly design jig and fixture for computer-assisted knee replacement surgery. Orthopedic surgeons found it difficult to align the existing jig with the computer-assisted device during the operation and it is time consuming to place it at the right position. A newly design jig and fixture has been proposed to solve this problem. Visualization is needed to show the surgeons on how it will be used in the computer-assisted knee replacement surgery. Virtual models used in this visualization are constructed from the actual equipment and real human dataset.

Intelligent Computer-Aided Training (ICAT) systems integrate artificial intelligence and simulation technologies to deliver training for complex, procedural tasks in a distributed, workstation-based environment. Computer applications of increasing diversity form a growing part of the undergraduate education of meteorologists in the early twenty-first century. The advent of the Internet economy, as well as a waning demand for traditional forecasters brought about by better numerical models and statistical forecasting techniques has greatly increased the need for operational and commercial meteorologists to acquire computer skills beyond the traditional techniques of numerical analysis and applied statistics. Specifically, students with the skills to develop data distribution products are in high demand in the private sector job market. Meeting these demands requires greater breadth, depth, and efficiency in computer instruction. The authors suggest that computer instruction for undergraduate meteorologists should include three key elements: a data distribution focus, emphasis on the techniques required to learn computer programming on an as-needed basis, and a project orientation to promote management skills and support student morale. In an exploration of this approach, the authors have reinvented the Applications of Computers to Meteorology course in the Department of Meteorology at The Pennsylvania State University to teach computer programming within the framework of an Internet product development cycle. Because the computer skills required for data distribution programming change rapidly, specific languages are valuable for only a limited time. A key goal of this course was therefore to help students learn how to retrain efficiently as technologies evolve. The crux of the course was a semester-long project during which students developed an Internet data distribution product. As project management skills are also important in the job market, the course teamed students in groups of four for this product development project. Specific autonomous training systems based on artificial intelligence technology for use by NASA astronauts, flight controllers, and ground-based support personnel that demonstrate an alternative to current training systems are described. In addition to these specific systems, the evolution of a general architecture for autonomous intelligent training systems that integrates many of the features of traditional training programs with artificial intelligence techniques is presented. These Intelligent Computer-Aided Training (ICAT) systems would provide, for the trainee, much of the same experience that could be gained from the best on-the-job training. By integrating domain expertise with a knowledge of appropriate training methods, an ICAT session should duplicate, as closely as possible, the trainee undergoing on-the-job training in the task environment, benefitting from the full attention of a task expert who is also an expert trainer. Thus, the philosophy of the ICAT system is to emulate the behavior of an experienced individual devoting his full time and attention to the training of a novice - proposing challenging training scenarios, monitoring and evaluating the actions of the trainee, providing meaningful comments in response to trainee errors, responding to trainee requests for information, giving hints (if appropriate), and remembering the strengths and weaknesses displayed by the trainee so that appropriate future exercises can be designed.

Traditional programming languages, authoring systems, and authoring languages are the three principal approaches to coding courseware, and each approach has advantages and disadvantages. The use of an authoring language or an authoring system may be the easiest courseware development method for the nurse educator who is unfamiliar with programming techniques. The computer-assisted diagnosis of melanoma is an exciting area of research where imaging techniques are combined with diagnostic algorithms in an attempt to improve detection and outcomes for patients with skin lesions suspicious for malignancy. Once an image has been acquired, it undergoes a processing pathway which includes preprocessing, enhancement, segmentation, feature extraction, feature selection, change detection, and ultimately classification. Practicality for everyday clinical use remains a vital question. A successful model must obtain results that are on par or outperform experienced dermatologists, keep costs at a minimum, be user-friendly, and be time efficient with high sensitivity and specificity. New simulation tools imply new opportunities to teach skills and train health care professionals. The aim of this study was to investigate the learning gained from computer simulation skills training. The study was designed for optimal educational settings, which benefit student-centred learning. Twenty-four second year undergraduate nursing students practised intravenous catheterization with the computer simulation program CathSim. Questionnaires were answered before and after the skills training, and after the skills examination. When using CathSim, the students appreciated the variation in patient cases, the immediate feedback, and a better understanding of anatomy, but they missed having an arm model to hold. We concluded that CathSim was useful in the students' learning process and skills training when appropriately integrated into the curriculum. Learning features to be aware of when organizing curricula with simulators are motivation, realism, variation, meaningfulness and feedback. PMID:20015690

This paper describes the basic technological concepts of modern computer assisted surgical systems. A description is presented of the approaches used to modify the molecular structure generator program, CONGEN. The CONGEN program for constructing structures under constraints has been discussed by Carhart et al. (1975). The modifications reported are to lead to a more efficient structure generation on the basis of a translation of structural data input to the program. From an algorithmic standpoint, CONGEN is successful if it can, in a reasonable amount of time and without exhausting storage resources, produce a list of candidate structures satisfying the chemist's constraints. However, this list is often quite large, and it remains for the chemist to discriminate among the candidates, eventually reducing the possibilities to just one structure. Ways are studied for providing computer assistance in examining and further constraining lists of structural candidates.

The prototype implementation of an expert system was developed to assist the user in the computer aided parallelization process. The system interfaces to tools for automatic parallelization and performance analysis. By fusing static program structure information and dynamic performance analysis data the expert system can help the user to filter, correlate, and interpret the data gathered by the existing tools. Sections of the code that show poor performance and require further attention are rapidly identified and suggestions for improvements are presented to the user. In this paper we describe the components of the expert system and discuss its interface to the existing tools. We present a case study to demonstrate the successful use in full scale scientific applications.

ABSTRACT The longevity of total knee prostheses depends mostly on the correct alignment (frontal, sagittal and axial) of the prosthetic components, soft tissue balancing and restoring the mechanical axis of the lower limb. The use of computer-assisted navigation allows more accurate and reproducible restoration of mechanical axis and component positioning, better results in patients with extra-articular deformities and it has an important role in surgical training. Better alignment should lead to an improved functional outcome and an increased long-term survival of the prosthesis. This courseware is different from most others because it does not simulate process control machinery or other computer programs, instead the WHC Excerises model day-to-day tasks such as physical work preparations, progress, and incident handling. Systems for automated image analysis are useful for a variety of tasks and their importance is still increasing due to technological advances and an increase of social acceptance. Especially in the field of driver assistance systems the progress in science has reached a level of high performance. Fully or partly autonomously guided vehicles, particularly for road-based traffic, pose high demands on the development of reliable algorithms due to the conditions imposed by natural environments. At the Institut fur Neuroinformatik, methods for analyzing driving relevant scenes by computer vision are developed in cooperation with several partners from the automobile industry. We introduce a system which extracts the important information from an image taken by a CCD camera installed at the rear view mirror in a car. The approach consists of a sequential and a parallel sensor and information processing. Three main tasks namely the initial segmentation (object detection), the object tracking and the object classification are realized by integration in the sequential branch and by fusion in the parallel branch. The main gain of this approach is given by the integrative coupling of different algorithms providing partly redundant information.

The FMA prototype is a ruggedized, water-resistant personal computer, weighing approximately 5 lbs. It has an LCD graphical user interface display for patient record input and output, pen-based and audio input, audio output, and wireless communications capabilities. Automatic recording and dynamic, graphical display of time-stamped trends in patient vital signs will be simulated during the demonstration. Means for accessing existing patient record information (e.g., allergies to particular medications) and updating the record with the nature of the injury, its cause, and the treatments that were administered will be shown. These will include use of an electronic pen to mark up anatoglyphs (standard drawings of human body appearing on computer screen) to show where injuries occurred and where treatments were applied, and to input textual descriptions of the nature of the injury, its cause, what treatments were administered, etc. Computer recognition of handwritten inputs will be shown. Likewise, voice annotation and audio playback of patient record information by medics and hospital personnel will be illustrated. These latter technologies free the care providers' hands to treat the patient; they can therefore provide inputs to the patient record while information is fresh in their minds. The audio playback option allows hospital personnel to select more detailed voice annotations of specific portions of the patient record by simply touching the electronic pen to a particular place where an electronic pen marking was made by a medic in the field and then listening to the medic's corresponding audio commentary. Finally, the FMA's means for assisting the medic in simultaneously managing several injured patients will be shown. (abstract truncated) PMID:8591545

Introduction: Total knee arthroplasty (TKA) has traditionally been and largely continues to be aligned mechanically, that being with a neutral coronal plane mechanical tibiofemoral axis and a joint line orientated at 900 to this axis. Femoral component rotation is set by gap balancing or by externally rotating 30 from any of a number femoral reference lines. This produces a rectangular flexion gap and relaxes patellar tracking. Kinematic alignment (KA) is an alternative technique that aims to restore premorbid alignment, joint orientation and ligament tension. The basic premise for this technique is based on evidence that the medial and lateral femoral condyles consistently equate to cylinders of equal or near equal size and that therefore with a fixed radius, cruciate retaining implant, matched distal femoral, posterior femoral and proximal tibial resections, accounting for bone and cartilage already lost will reproduce the premorbid joint line and restore native premorbid kinematics. Femoral rotation is therefore referenced off the prearthritic posterior condylar axis (PCA) that is on average internally rotated to the AP axis. Kinematic alignment therefore has the potential to challenge patellar tracking, increase patellar load and potentially increase patellar complications. Method: Case control study – level of evidence III-2. Between November 2012 and June 2013 the senior author completed 104 consecutive computer assisted (CAS) kinematically aligned total knee arthroplasties (TKA) with a cruciate retaining, fixed bearing, single radius implant. For the better part of the past 20 years, dentistry has seen the development of many new all-ceramic materials and restorative techniques fueled by the desire to capture the ever elusive esthetic perfection. This has resulted in the fusion of the latest in material science and the pen ultimate in computer-assisted design/computer-assisted manufacturing (CAD/CAM) technology. This case report describes the procedure for restoring the esthetic appearance of both the left and right maxillary peg-shaped lateral incisors with a metal-free sintered finely structured feldspar ceramic material using the latest laboratory CAD/CAM system. The use of CAD/CAM technology makes it possible to produce restorations faster with precision- fit and good esthetics overcoming the errors associated with traditional ceramo-metal technology. There has been relatively little discussion in the literature regarding the role of the healthcare assistant (HCA) in midwifery. The study examines and contrasts HCA duties in one maternity department in the Republic of Ireland. Observations of six trained HCAs were undertaken to evaluate if they have the relevant repertoire of skills as taught in a national training programme, and to assess if they are using these skills competently. Findings indicate that HCAs carried out more indirect patient care activities than direct care activities. Findings show that the HCA provided support to qualified staff by assisting them in a range of care activities. This was consistent with the specified job description and with their training programme. This exploratory study helps to further understand the role of the HCA. It identifies the activities that HCAs undertake in support of qualified staff in maternity settings. The findings highlight the need for further investigation to determine the relationship between the role of the trained HCA and the qualified midwife. PMID:15732618

Objectives To evaluate the feasibility and efficacy of a handheld personal digital assistant (PDA)-based supplement for maternal Problem-Solving Skills Training (PSST) and to explore Spanish-speaking mothers’ experiences with it. Methods Mothers (n = 197) of children with newly diagnosed cancer were randomized to traditional PSST or PSST + PDA 8-week programs. Participants completed the Social Problem-Solving Inventory-Revised, Beck Depression Inventory-II, Profile of Mood States, and Impact of Event Scale-Revised pre-, post-treatment, and 3 months after completion of the intervention. Mothers also rated optimism, logic, and confidence in the intervention and technology. Results Both groups demonstrated significant positive change over time on all psychosocial measures. No between-group differences emerged. Despite technological “glitches,” mothers expressed moderately high optimism, appreciation for logic, and confidence in both interventions and rated the PDA-based program favorably. Technology appealed to all Spanish-speaking mothers, with younger mothers showing greater proficiency. Conclusions Well-designed, supported technology holds promise for enhancing psychological interventions. PMID:19091804

Computer-based memory and attention training methods improve episodic recall in older adults who have amnestic mild cognitive impairment.(1,2) Memory and attention are highly interactive and interdependent processes due to their shared circuitry. The cognitive benefits of computer-based memory training appear to persist for at least 6 months.(1) Traditional cognitive training programs are administered by professionals and may cost as much as $15 to $100 an hour, depending on the educational level of the staff member who delivers the training.(2) More cost-effective methods of computer-based memory training are needed. The aim of the longitudinal study was to investigate whether a computer application designed for remedial reading training can enhance letter knowledge, reading accuracy, fluency, and spelling of at-risk children. The participants, 7-year-old Finnish school beginners (N=166), were assigned to 1 of 3 groups: (a) regular remedial reading intervention (n=25), (b) computer-assessed reading intervention (n=25), and (c) mainstream reading instruction (n=116). Based on the results, computer-assisted remedial reading intervention was highly beneficial, whereas regular type of intervention was less successful. The results indicated that at-risk children require computer-based letter-name and letter-sound training to acquire adequate decoding and spelling skills, and to reach the level of their non-at-risk peers. Background Despite the growing diffusion of robotic devices in neurorehabilitation, no previous study investigated the effects of robotic training on arm impairment due to Parkinson’s disease. The aim of this pilot study was to evaluate whether robot-assisted arm training might improve upper limb function in patients with Parkinson’s disease. Findings Ten patients with Parkinson’s disease (Hoehn and Yahr stage 2.5-3) received ten, 45-minute, treatment sessions, five days a week, for two consecutive weeks. Robot-assisted arm training was performed with the Bi-Manu-Track (Reha-Stim, Berlin, Germany) that provides a computer-controlled, repetitive, bilateral, mirror-like practice of forearm pronation/supination and wrist extension/flexion. Patients were trained according to the following modalities: passive-passive (both arms moved by the machine) and active-active (both arms actively moving against resistance). The dominant upper limb was evaluated before and immediately after treatment as well as at two weeks of follow-up. Outcomes were the nine-hole peg test, the Fugl-Meyer assessment (upper limb section) and the Unified Parkinson’s Disease Rating Scale. After treatment, a significant improvement was found in the nine-hole peg test (P = 0.007) as well as in the upper limb section of the Fugl-Meyer assessment (P = 0.012). Findings were confirmed at the 2-week follow-up evaluation only for the nine-hole peg test (P = 0.007). No significant improvement was found in the Unified Parkinson’s Disease Rating Scale at both post-treatment and follow-up evaluations. Conclusions Our findings support the hypothesis that robot-assisted arm training might be a promising tool in order to improve upper limb function in patients with Parkinson’s disease. PMID:24597524

The combination of interstitial radiotherapy and interstitial hyperthermia is more promising in the treatment of tumors than one of these methods alone. The unit developed by us uses the afterloading needles for heating up the tumor tissue with ohm current and for controlling the distribution of temperature in the target volume. Up to twelve needles are provided by one commutator with the R.F. current controlled by the computer. The temperature is measured by three thermistors per needle which are arranged at an axial distance of 2 cm each. The linearization of the thermistor characteristics and the control of cummutator and R.F. generator is performed by the computer over an interface constructed by us. In order to achieve a homogeneous distribution of temperature in the target volume and to avoid hot spots, we have examined several needle configurations by measuring in an homogeneous phantom. In general the software required by a blast designer includes tools that graphically present blast designs (surface and underground), can analyze a design or predict its result, and can assess blasting results. As computers develop and computer literacy continues to rise the development of and use of such tools will spread. An example of the tools that are becoming available includes: Automatic blast pattern generation and underground ring design; blast design evaluation in terms of explosive distribution and detonation simulation; fragmentation prediction; blast vibration prediction and minimization; blast monitoring for assessment of dynamic performance; vibration measurement, display and signal processing; evaluation of blast results in terms of fragmentation; and risk and reliability based blast assessment. The authors have identified a set of criteria that are essential in choosing appropriate software blasting tools.
