Some sources include the Japanese-American Internee File, the Community Action Program Grant Master Files, and the Civil War Sites Database. From the homepage, visitors can learn about the architecture of the database, read about the terminology used on the site (and in the archives more generally), and how to get help using the archives. To begin, users will want to select a file unit, which are divided by subject, geography, organization, time span, and creator (such as civilian or military agency). After making this selection, a list of relevant file units will be returned, and users can select the one that is most germane to their area of interest. Identification of Rheumatoid Arthritis Patients Using an Administrative Database: A Veterans Affairs Study

The accuracy of the diagnosis is vital when administrative databases are used for pharmacoepidemiologic and outcome studies. Data pertaining to the utility of databases for rheumatoid arthritis (RA) is sparse and variable. We assessed the utility of various diagnostic algorithms to identify RA patients within the Veterans Health Administration (VHA) databases. Using the ICD code for RA at two visits, at least six months apart, we identified 1779 patients between 10/1/1998 and 9/30/2009 in our local Veteran Affairs Medical Center administrative database. Disease Modifying Anti Rheumatic Drugs (DMARD) use was ascertained from the pharmacy database. Cases were analyzed based on DMARD therapy and RA codes at clinic visits. 543 patients' medical records selected by stratification and random selection on the basis of their visits were reviewed to ascertain the clinicians' diagnoses and clinical criteria documentation. Positive predictive values (PPV) were calculated for various database case identification algorithms using diagnosis of RA by medical record review as the gold standard. PPV for identification of RA with two RA codes six months apart was 30.9%. Addition of DMARD therapy increased the PPV to 60.4%. The PPV further increased to 91.4% when having RA code at the last VAMC rheumatology clinic visit criterion was added. An algorithm using only two administrative RA codes six months apart had a low PPV for correctly identifying patients with RA in the VHA database. Including DMARD therapy and requiring a RA code at the last visit with a rheumatologist increased the performance of the data extraction algorithm.

Validity of ascertainment of co-morbid illness using administrative databases: a systematic review.

Co-morbid illnesses have a major influence on the epidemiology of infectious diseases. Although International Classification of Diseases (ICD) discharge codes are frequently used to evaluate the presence of co-morbidities in observational research, additional research is required about their validity. We reviewed the evidence supporting the use of routinely coded administrative data for ascertainment of co-morbid diseases with emphasis as it relates to the study of infectious diseases. A systematic Medline, Embase, and bibliographic review were conducted in order to identify and critically appraise published (1990-2008) studies comparing administrative databases with conventional chart review. Twenty-one co-morbidities commonly associated with infectious diseases risk were a priori selected for specific evaluation. Of the 21 co-morbid conditions chosen, only 19 had adequate data available for evaluation. Thirteen studies were included; only one focused on an infectious disease population. Eleven articles validated individual co-morbid conditions data in electronic administrative databases and reported a wide range of pooled sensitivity (13-82%) but overall high pooled specificity (>97%) when compared with medical chart review. Seven articles compared Charlson Co-morbidity Index scores derived from administrative data algorithms as compared with that calculated from medical record review and found that administrative data underscored the index in all articles with kappa agreement ranging from 0.30 to 0.56. The small body of literature published to date suggests that electronic administrative databases have limited validity for co-morbidity evaluation. Studies evaluating administrative database ascertainment of co-morbidities specifically in infectious diseases research are needed. PMID:19614717

Developing model-based algorithms to identify screening colonoscopies using administrative health databases

Background Algorithms to identify screening colonoscopies in administrative databases would be useful for monitoring colorectal cancer (CRC) screening uptake, tracking health resource utilization, and quality assurance. Previously developed algorithms based on expert opinion were insufficiently accurate. The purpose of this study was to develop and evaluate the accuracy of model-based algorithms to identify screening colonoscopies in health administrative databases. Methods Patients aged 50-75 were recruited from endoscopy units in Montreal, Quebec, and Calgary, Alberta. Physician billing records and hospitalization data were obtained for each patient from the provincial administrative health databases. Indication for colonoscopy was derived using Bayesian latent class analysis informed by endoscopist and patient questionnaire responses. Two modeling methods were used to fit the data, multivariate logistic regression and recursive partitioning. The accuracies of these models were assessed. Results 689 patients from Montreal and 541 from Calgary participated (January to March 2007). The latent class model identified 554 screening exams. Multivariate logistic regression predictions yielded an area under the curve of 0.786. Recursive partitioning using the latent outcome had sensitivity and specificity of 84.5% (95% CI: 81.5-87.5) and 63.3% (95% CI: 59.7-67.0), respectively. Conclusions Model-based algorithms using administrative data failed to identify screening colonoscopies with sufficient accuracy. Nevertheless, the approach of constructing a latent reference standard against which model-based algorithms were evaluated may be useful for validating administrative data in other contexts where there lacks a gold standard.

Using administrative databases in the surveillance of depressive disorders--case definitions.

The objective of this study was to assess the usefulness of provincial administrative databases in carrying out surveillance on depressive disorders. Electronic medical records (EMRs) at 3 family practice clinics in St. John's, NL, Canada, were audited; 253 depressive disorder cases and 257 patients not diagnosed with a depressive disorder were selected. The EMR served as the "gold standard," which then was compared to these same patients investigated through the use of various case definitions applied against the provincial hospital and physician administrative databases. Variables used in the development of the case definitions were depressive disorder diagnoses (either in hospital or physician claims data), date of diagnosis, and service provider type [general practitioner (GP) vs. psychiatrist]. Of the 120 case definitions investigated, 26 were found to have a kappa statistic greater than 0.6, of which 5 case definitions were considered the most appropriate for surveillance of depressive disorders. Of the 5 definitions, the following case definition, with a 77.5% sensitivity and 93% specificity, was found to be the most valid ([??1 hospitalizations OR ?1 psychiatrist visit related to depressive disorders any time] OR ?2 GP visits related to depressive disorders within the first 2 years of diagnosis). This study found that provincial administrative databases may be useful for carrying out surveillance on depressive disorders among the adult population. Nursing leadership succession planning in Veterans Health Administration: creating a useful database.

An electronic database was developed for succession planning and placement of nursing leaders interested and ready, willing, and able to accept an assignment in a nursing leadership position. The tool is a 1-page form used to identify candidates for nursing leadership assignments. This tool has been deployed nationally, with access to the database restricted to nurse executives at every Veterans Health Administration facility for the purpose of entering the names of developed nurse leaders ready for a leadership assignment. The tool is easily accessed through the Veterans Health Administration Office of Nursing Service, and by limiting access to the nurse executive group, ensures candidates identified are qualified. Demographic information included on the survey tool includes the candidate's demographic information and other certifications/credentials. This completed information form is entered into a database from which a report can be generated, resulting in a listing of potential candidates to contact to supplement a local or Veterans Integrated Service Network wide position announcement. The data forms can be sorted by positions, areas of clinical or functional experience, training programs completed, and geographic preference. The forms can be edited or updated and/or added or deleted in the system as the need is identified. This tool allows facilities with limited internal candidates to have a resource with Department of Veterans Affairs prepared staff in which to seek additional candidates. In addition to traditional clinical trials, practitioners are heeding results of high-quality comparative-effectiveness and health services research analyses to better understand the complexity of disease epidemiology, care variation, health care costs, and surgical utilization for common conditions. Many national publicly available data sources exist for use in clinical research that may be of benefit for the academic and clinical otolaryngologist. Inappropriate medication use among the elderly: a systematic review of administrative databases

Background Inappropriate medication use (IMU) by elderly people is a public health problem associated with adverse effects on health. There are a number of methods for identifying IMU, some involving clinical judgment and others, consensually generated lists of drugs to be avoided. This review aims to describe studies that used information from insurance company and social security administrative databases to assess IMU among community-dwelling elderly and to present the risk factors most often associated with IMU. Methods The paper search was conducted in Medline and Embase, using descriptors combined with free terms in the title or abstract. The limits applied were: publication date from January 1990 to June 2010, species (human) and publication type (excluding editorials, letters and reviews). Excluded were: case studies; studies in hospitals, nursing homes, or hospital emergency departments; studies of specific drugs or groups of drugs; studies exclusively of subgroups of ill, frail elderly or rural populations. Additional studies were identified from reference lists. Data were selected and extracted after independent reading by two of the authors, with disagreements resolved by a third author. The primary outcome assessed was prevalence of IMU, defined as the proportion of elderly who received at least one inappropriate medication. Results Of the 628 studies, 19 met the inclusion criteria, 78.9% of them conducted in the USA. All papers included used explicit criteria of inappropriateness, most commonly Beers criteria (73.7%) in their three versions (1991, 1997 and 2002). Other methods used included Zhan, which is derived from on Beers criteria and was applied in 21% of the papers selected. The study found that prevalence of IMU ranged from 11.5% to 62.5%. Only 68.4% of the studies included examined inappropriate use-related factors, the most important being female sex, advanced age and larger number of drugs. Conclusions The results show that the prevalence of IMU among community-dwelling elderly is high and depends partly on the method used to evaluate improper use. Besides the diversity of methods, other factors, such as patient sex, age and number of drugs used concurrently, appear to have influenced the estimates of IMU.

The care processes for acute cholecystitis according to clinical practice guidelines based on the Japanese administrative database.

Acute cholecystitis is one of the most frequently encountered conditions in daily practice in Japan. However, there is a shortage of detailed data about treatments that have been performed according to the clinical practice guidelines (CPGs) for acute cholecystitis. We therefore examined the management of acute cholecystitis for adherence to the appropriate CPGs using the Japanese administrative database associated with the Diagnosis Procedure Combination (DPC) system. We collected data from 6,070 patients with acute cholecystitis, examining for the application of four recommended treatments (administration of antimicrobial drugs and nonsteroidal anti-inflammatory drugs (NSAIDs) and performance of early and laparoscopic cholecystectomy). The patients were classified according to the procedures documented for each case: no gallbladder drainage (n = 4,333), gallbladder drainage without supportive care (ventilation or hemodiafiltration or the use of vasopressor) (n = 1,591) and gallbladder drainage and supportive care (n = 146). Multiple logistic regression models revealed that patients with gallbladder drainage without supportive care and those with gallbladder drainage and supportive care significantly higher received administration of antimicrobial drugs and NSAIDs, while these patients underwent less early or laparoscopic cholecystectomy than did patients without gallbladder drainage, after adjusting for potential confounding effects of the clinical variables. This study demonstrated that there were various differences with regard to the performance of recommended treatments between the levels of procedures required for acute cholecystitis. In addition, this administrative database was a feasible tool for the evaluation of care processes and will provide useful information contributing to improved quality of medical care. PMID:22820613

Reoperation, as a surrogate measure for poor outcome, may be biased by preferences of patients and surgeons and may even be planned a priori. Other available administrative data outcomes, such as postoperative cross sectional imaging (PCSI), may better reflect changes in functional outcome. The purpose was to determine if postoperative events captured from administrative databases, namely reoperation and PCSI, reflect outcomes as derived by validated functional outcome measures (short form 36 scores, Oswestry disability index) for patients who underwent discretionary surgery for specific degenerative conditions of the lumbar spine such as disc herniation, spinal stenosis, degenerative spondylolisthesis, and isthmic spondylolisthesis. After reviewing the records of all patients surgically treated for disc herniation, spinal stenosis, degenerative spondylolisthesis, and isthmic spondylolisthesis at our institution, we recorded the occurrence of PCSI (MRI or CT-myelograms) and reoperations, as well as demographic, surgical, and functional outcome data. We determined how early (within 6 months) and intermediate (within 18 months) term events (PCSI and reoperations) were associated with changes in intermediate (minimum 1 year) and late (minimum 2 years) term functional outcome, respectively. We further evaluated how early (6-12 months) and intermediate (12-24 months) term changes in functional outcome were associated with the subsequent occurrence of intermediate (12-24 months) and late (beyond 24 months) term adverse events, respectively. From 148 surgically treated patients, we found no significant relationship between the occurrence of PCSI or reoperation and subsequent changes in functional outcome at intermediate or late term. Similarly, earlier changes in functional outcome did not have any significant relationship with subsequent occurrences of adverse events at intermediate or late term. Although it may be tempting to consider administrative database outcome measures as proxies for poor functional outcome, we cannot conclude that a significant relationship exists between the occurrence of PCSI or reoperation and changes in functional outcome. Review of Reported Clinical Information System Adverse Events in US Food and Drug Administration Databases

Background The US FDA has been collecting information on medical devices involved in significant adverse advents since 1984. These reports have been used by researchers to advise clinicians on potential risks and complications of using these devices. Objective Research adverse events related to the use of Clinical Information Systems (CIS) as reported in FDA databases. Methods Three large, national, adverse event medical device databases were examined for reports pertaining to CIS. Results One hundred and twenty unique reports (from over 1.4 million reports) were found, representing 32 manufacturers. The manifestations of these adverse events included: missing or incorrect data, data displayed for the wrong patient, chaos during system downtime and system unavailable for use. Analysis of these reports illustrated events associated with system design, implementation, use, and support. Conclusion The identified causes can be used by manufacturers to improve their products and by clinical facilities and providers to adjust their workflow and implementation processes appropriately. The small number of reports found indicates a need to raise awareness regarding publicly available tools for documenting problems with CIS and for additional reporting and dialog between manufacturers, organizations, and users.

Intussusception among Japanese children: an epidemiologic study using an administrative database

Background The epidemiology of intussusception, including its incidence, can vary between different countries. The aim of this study was to describe the epidemiology of childhood intussusception in Japan using data from a nationwide inpatient database. Methods We screened the database for eligible cases ? 18 years of age, who were coded with a discharge diagnosis of intussusception (International Classification of Diseases, 10th revision: K-561) between July to December in 2007 and 2008. We then selected cases according to Level 1 of the diagnostic certainty criteria developed by the Brighton Collaboration Intussusception Working Group. We examined the demographics, management, and outcomes of cases, and estimated the incidence of intussusception. Results We identified 2,427 cases of intussusception. There were an estimated 2,000 cases of infantile intussusception annually in Japan, an incidence of 180-190 cases per 100,000 infants. The median age at diagnosis was 17 months, and two-thirds of the patients were male. Treatment with an enema was successful in 93.0% of cases (2255/2427). The remainder required surgery. Secondary cases accounted for 3.1% (76/2427). Median length of hospital stay was 3 days. Of the 2,427 cases, we found 2 fatal cases associated with intussusception. Conclusions This is currently the largest survey of childhood intussusception in Asia using a standardized case definition. Our results provide an estimate of the baseline risk of intussusception in Japan, and it is higher than the risk observed in other countries.

Antifracture drugs consumption in Tuscany Region before the Target Project: a valuable model for the analysis of administrative database

Summary Objectives As the Target Project of Tuscany Region is aimed to identify significant changes occurring during the time of the project compared to the previous years, we have analyzed antifracture drugs consumption in the years immediately preceding the start-up of the program. Methods We have analyzed the regional administrative database to search for antifracture drugs delivered by the regional healthcare system to people living in Tuscany in the year 2009. Both drugs directly distributed by hospitals or local health authorities (direct distribution dataset, FED), and those delivered through pharmacies (pharmaceutical distribution dataset, SPF) were included in the analysis. Private expenditures were indirectly computed because total pharmaceutical expenditures were also known. A specific subgroup analysis has been carried out on elderly patients with femoral fractures (years 20062010), which represent the population of Target Project, in order to assess the evolution of all antifracture drug prescriptions before the start up of the project. Results A total of 690,768 boxes of antifracture drugs (20 million defined daily doses, DDD) have been delivered in year 2009 for total expenditures of 23 million Euros. Public expenditures accounted for 71% of the total (16 million; 6% of regional pharmaceutical expenditures). Alendronate and risedronate were the most used drugs. The number of antifracture medications prescribed within the first 90 days to elderly people with hip fractures increased in the period before the start-up of the Target project but they remained under 14%. Conclusions There is the need for an appropriate management of antifracture therapies, as they represent a valuable proportion of pharmaceutical expenditures. This need is pointed out also by the remarkable amount of direct private expenditures and by the few number of hip fractured people starting a treatment.

The priority-based assessment of food additives (PAFA) is a database maintained by the U.S. Food and Drug Administration (FDA) Center for Food Safety and Applied Nutrition. PAFA contains extensive administrative, chemical, and toxicological information on 1685 regulated direct food additives. The database also has limited administrative and chemical information on an additional 1236 direct additives. The total 2921 substances represent everything added to food in the United States. PAFA contains up to 150 different kinds of information about each chemical. Administrative and chemical information includes Chemical Abstracts Service Registry numbers, Code of Federal Regulations citations, the annual usage and estimated daily U.S. human consumption, the Joint Committee on Food Additives Allowable Daily Intakes, the FDA Redbook structure categories of the chemicals, and their technical effects. Toxicology information shows the type of studies done for each chemical, the species of animals tested, the toxicological effects observed and the sites where they were seen, the lowest doses that cause a toxicological effect in each study, a source citation, and other types of related information.

Removing user fees for basic health services: a pilot study and national roll-out in Afghanistan

Background User fees for primary care tend to suppress utilization, and many countries are experimenting with fee removal. Studies show that additional inputs are needed after removing fees, although well-documented experiences are lacking. This study presents data on the effects of fee removal on facility quality and utilization in Afghanistan, based on a pilot experiment and subsequent nationwide ban on fees. Methods Data on utilization and observed structural and perceived overall quality of health care were compared from before-and-after facility assessments, patient exit interviews and catchment area household surveys from eight facilities where fees were removed and 14 facilities where fee levels remained constant, as part of a larger health financing pilot study from 2005 to 2007. After a national user fee ban was instituted in 2008, health facility administrative data were analysed to assess subsequent changes in utilization and quality. Results The pilot study analysis indicated that observed and perceived quality increased across facilities but did not differ by fee removal status. Difference-in-difference analysis showed that utilization at facilities previously charging both service and drug fees increased by 400% more after fee removal, prompting additional inputs from service providers, compared with facilities that previously only charged service fees or had no change in fees (P = 0.001). Following the national fee ban, visits for curative care increased significantly (P < 0.001), but institutional deliveries did not. Services typically free before the banimmunization and antenatal carehad immediate increases in utilization but these were not sustained. Conclusion Both pilot and nationwide data indicated that curative care utilization increased following fee removal, without differential changes in quality. Concerns raised by non-governmental organizations, health workers and community leaders over the effects of lost revenue and increased utilization require continued effort to raise revenues, monitor health worker and patient perceptions, and carefully manage health facility performance.

Validation of physician billing and hospitalization data to identify patients with ischemic heart disease using data from the Electronic Medical Record Administrative data Linked Database (EMRALD)

BACKGROUND: Reporting of ischemic heart disease (IHD) prevalence in Canada has been based on self-report or patients presenting to hospital. However, IHD often presents and can be managed in the outpatient setting. OBJECTIVES: To determine whether the combination of hospital data and physician billings could accurately identify patients with IHD. METHODS: A random sample of 969 adult patients from the Electronic Medical Record Administrative data Linked Database (EMRALD) an electronic medical record database of primary care physicians in Ontario linked to administrative data for the province of Ontario was used. A number of combinations of physician billing and hospital discharge abstracts were tested to determine the accuracy of using administrative data to identify IHD patients. RESULTS: Two physician billings within a one-year period (with one of the billings by a specialist or a family physician in a hospital or emergency room setting) or a hospital discharge abstract gave a sensitivity of 77.0% (95% CI 68.2% to 85.9%), a specificity of 98.0% (95% CI 97.0% to 98.9%), a positive predictive value of 78.8% (95% CI 70.1% to 87.5%), a negative predictive value of 97.7% (95% CI 96.8% to 98.7%) and a kappa of 0.76 (95% CI 0.68 to 0.83). CONCLUSIONS: A combination of physician billing and hospital discharge abstracts can be used to identify patients with IHD. Population prevalence of IHD can be measured using administrative data.

Comparing Shingles Incidence and Complication Rates From Medical Record Review and Administrative Database Estimates: How Close Are They?

Accurate rates of herpes zoster incidence and complication have become of greater interest as studies have suggested an increasing temporal trend in incidence rates across all age groups and long-term follow-up studies of vaccine effectiveness are required by the Food and Drug Administration. This study compares the results obtained from the most commonly used method to obtain herpes zoster data (rates obtained from administrative data) with results obtained when administrative data are supplemented by medical record review. Administrative billing code data identified 1,959 cases of herpes zoster in Olmsted County, Minnesota, adults between January 1, 1996, and December 31, 2001. Of those 1,959 cases, 1,669 (85.2%) could be confirmed by medical record review, a decrease in incidence rate of 14.8%, resulting in a decrease of 0.61/1,000 person-years when adjusted to the US adult population. Complication rates were also significantly different between the 2 methods. It is not clear if the 15% decrease in incidence rates would be seen in every administrative data set or if the lack of confirmation of cases may be variable in both validity and reproducibility between data sets, making estimations in temporal trends and pre/post-vaccine rates difficult to compare across data resources.

The prevalence of common illnesses could be estimated using general practice databases, providing certain advantages when compared to other alternative sources of information, in particular being relatively more cost-effective. The main limitation is that it is a threat of selection bias. Some individuals have a higher probability of having used primary health care, implying that the potential result, 'contact registration', is overrepresented in the sample observed. The selection bias would provide inconsistent estimators of prevalence. The objective of this study is to propose a bias-free selection method to estimate prevalence using an administrative primary health care database. It proposes re-weighting the estimations of prevalence obtained from the database according to the probability of their being present in the same. These probabilities will be appropriately estimated from a health survey using a treatment effects model with a discrete response, i.e. a hurdle model. As an application, it was estimated the prevalence of hypertension in the population covered by public primary health care providers in the Girona Health Region, Spain, in 2005. Using this bias-free selection method the prevalence of hypertension has been estimated that 15.5% of individuals aged 15 and above (14.1% among men and 16.9% among women) suffer from hypertension. Likewise, the prevalence is estimated at 31.1% (30.3% men and 32.0% women) in individuals aged 45 and over; 48.3% (44.1% men and 51.9% women) among those aged 65 and over; and 13.1% (11.8% men and 13.9% women) in the general population. The proposed method provides estimators of the prevalence of hypertension very close to those obtained directly from the 2006 Catalan Health Survey. It is concluded that the proposed method could be used to estimate the prevalence of hypertension in an approximately unbiased form. Given the use of the administrative primary health care database corresponding to all users of primary health care in the 23 public managed Health Areas of the Girona Health Region during 2005, the proposed method will be more cost-effective and will provide much more population information than health questionnaires. Intervenors in utility-law litigation are not generally entitled to collect attorney's fees, but some courts and Commissions are making exceptions. Court cases involving the Greene County Planning Board and the Power Authority of the State of New York illustrate the changes that are taking place on this question. Two state supreme courts, one in Nevada and one in California, have also affirmed a policy for reimbursement. New regulations were proposed by the Federal Communications Commission (FCC) in December of 1979 to set up a one-year pilot reimbursement program for public litigant's fees in FCC proceedings. Comments on the FCC proposal noted that a reimbursement program would neither be aburden nor disruptive. The FCC sought congressional appropriation to get around the problem of whether it had the authority to set up such a program. Dissenting comments emphasize the danger of underwriting self-serving private goals rather than the public interest. (DCK)

Given the severity of the condition, drug-induced anaphylaxis is a major public health concern. We developed and evaluated the positive predictive value (PPV) of algorithms for identifying anaphylaxis in the Mini-Sentinel Distributed Database. Methods We conducted a retrospective study among members enrolled in 8 geographically diverse health plans (HealthCore, Inc.; Humana; three member health plans within the Kaiser Permanente Center for Effectiveness and Safety Research; and two member health plans in the HMO Research Network). Diagnosis and procedure codes were used to identify potential cases of anaphylaxis recorded in the Mini-Sentinel Distributed Database between January 1, 2009 and December 31, 2010. A random sample of medical charts (n = 131) was abstracted and adjudicated and positive predictive values (PPVs) were calculated based upon diagnostic criteria by Sampson et al. Results Of the 131 potential available cases, adjudicators determined that 77 were anaphylaxis, 45 were not anaphylaxis, and 9 were determined to have inadequate information to confirm whether or not a case was anaphylaxis. Overall, the PPV was 63.1% (95% confidence interval, 53.9%71.7%). PPVs ranged from 48.1% to 78.9% across the Data Partners. In comparing different criteria, algorithms which included codes for allergy (ICD-9-CM 995.3) or adverse effects of drug, medicinal and biological substance (ICD-9-CM 995.2, E930-E949) in combination with additional treatment or symptom codes, such as injection of epinephrine, had a lower PPV (PPV = 45.8%) than algorithms that included codes specifically indicating anaphylaxis (ICD-9-CM 995.0, 999.4), although confidence intervals were wide and overlapped. Conclusions While the PPV of the developed algorithm was higher than those reported in previously published studies, the PPV of anaphylaxis remains low. Further evaluation of individual codes and combinations of codes is ongoing as part of the present study.

Financial barriers are an important obstacle for access to emergency obstetric care and a contributing factor to too slow a reduction in the level of maternal mortality. In Morocco, in 2009, a fee exemption policy for delivery and caesarean section was implemented in public maternity hospitals. As in most countries where a fee exemption policy has been implemented, fee exemption is considered synonym to free care. However, other direct costs may subsist. The objective of this study was to get an estimate of the actual cost of caesarean sections from the patients' perspective.This study was carried out in April 2010 in the three public hospitals in Fez. We carried out semi-structured interviews among a sample of 100 women who gave birth by caesarian section in the public hospitals in Fez. The results showed that households paid between US$169 (95% Confidence Interval (CI): 153, 185) at the provincial and regional hospitals, and US$291 (95% CI: 224-359) at the university hospital (UH) where the fee exemption was not applied. The direct cost of a caesarean was mainly influenced by the price of the drugs the families bought, the invoice paid at UH, and the transport. Finally, although the fee exemption policy for caesareans has probably reduced the total cost for households who did not have access to a poverty card, it has not led to 'truly' free caesarean deliveries. Allergies represent a significant medical and industrial problem. Molecular and clinical data on allergens are growing exponentially and in this article we have reviewed nine specialized allergen databases and identified data sources related to protein allergens contained in general purpose molecular databases. An analysis of allergens contained in public databases indicates a high level of redundancy of entries and a relatively low coverage of allergens by individual databases. From this analysis we identify current database needs for allergy research and, in particular, highlight the need for a centralized reference allergen database. PMID:14616118

Effective with any manuscript submitted after 31 December 2010, AGU is eliminating color charges in all formats. At the same time that we eliminate color charges, we are simplifying publication fees. Articles published in Journal of Geophysical Research; Geochemistry, Geophysics, Geosystems; and Radio Science will each have a flat fee of $1000, and Geophysical Research Letters (GRL) articles will each have a flat fee of $500. These prices represent a small drop from the average cost currently paid by authors. We will retain the excess page charges for full articles longer than 13 pages (including Water Resources Research) and 4 pages for letters, comments, and replies. This simplification means we can streamline the calculator authors must use when submitting manuscripts to AGU.

Chiropractic fee negotiations in Saskatchewan utilize the Chiropractic Compensation Review Committee with recourse to the Chiropractic Consultation Committee. Health care professionals who practise on a fee for service basis provide the government with a budgetary problem. Although the fees are set, the health care provider can determine his own income by deciding how many visit services he/she wishes to provide. In the fiscal years 1981-82 to 1990-91, chiropractors earned $699.00 per year more than one would expect given the increases in fee schedules. Each chiropractor earned $2,329.00 per year more than was necessary to make up for losses due to inflation. The allegation that unnecessary treatments were performed on patients is countered by analysis of the services per discrete patient values by mode of practice. The increased earnings of chiropractors was accomplished by treating an increasing percentage of the population who sought health care. Comparative information was obtained from the four western provinces.

The burden of epiglottitis among Japanese children before the Haemophilus influenzae type b vaccination era: an analysis using a nationwide administrative database.

Epiglottitis is a potentially life-threatening disease and is largely preventable by vaccination against Haemophilus influenzae type b (Hib). Little is known, however, about the epidemiology of childhood epiglottitis in Asian countries, including Japan. Using a nationwide inpatient database, this study aimed to determine the burden of childhood epiglottitis before the introduction of Hib vaccine into Japan. The study period was between July and December in 2007 and 2008, when Hib vaccine was not available. We found 102 cases with epiglottitis among children ?5years old. The annual incidence of epiglottitis in children ?5years old was estimated to be 3.2 per 100,000 population per year. Among the 102 patients, 31 (30.4%) required respiratory support, including two cases with tracheotomy and one fatal case. Our study demonstrated the substantial burden of epiglottitis among Japanese children, highlighting that a routine Hib vaccination program is essential. PMID:23512615

The DPD is a global list of plant databases including Taxonomic databases ("with systematic information on families or genera, or for Flora projects"); Collection catalogs (usually of herbaria); and DELTA datasets (DELTA is "the Description Language for Taxonomy, a data format for character data, used for identification, key construction and the generation of descriptions."). Imported foods must meet the same U.S. Food and Drug Administration (FDA) standards as domestic foods. The FDA determines whether an imported food is in compliance with the Federal Food, Drug, and Cosmetic Act. Pursuant to its regulatory activities, the FDA conducts compliance surveillance on imported foods offered for entry into the U.S. commerce. The National PulseNet Database is the molecular surveillance network for foodborne infections and is widely used to provide real-time subtyping support to epidemiologic investigations of foodborne diseases. FDA laboratories use pulsed-field gel electrophoresis to subtype foodborne pathogens recovered from imported foods and submit the molecular patterns to the National PulseNet Database at the Centers for Disease Control and Prevention. There were 60 isolates of Listeria monocytogenes in the FDA Field Accomplishment and Compliance Tracking System from 2001 to 2008 due to cheese imported from the following countries: Mexico (n=21 isolates), Italy (19), Israel (9), Portugal (5), Colombia (3), Greece (2), and Spain (1). We observed genetic diversity of L. monocytogenes isolates and genetic relatedness among strains recovered from imported cheese products coming to the United States from different countries. Consistent characterization of L. monocytogenes isolates recovered from imported cheeses, accompanied by epidemiologic investigations to ascertain human illness associated with these strains, could be helpful in the control of listeriosis acquired from imported cheeses. Since the Genome Project began several years ago, a plethora of databases have been developed or are in the works. They range from the massive Genome Data Base at Johns Hopkins University, the central repository of all gene mapping information, to small databases focusing on single chromosomes or organisms. Some are publicly available, others are essentially private electronic lab notebooks. Still others limit access to a consortium of researchers working on, say, a single human chromosome. An increasing number incorporate sophisticated search and analytical software, while others operate as little more than data lists. In consultation with numerous experts in the field, a list has been compiled of some key genome-related databases. The list was not limited to map and sequence databases but also included the tools investigators use to interpret and elucidate genetic data, such as protein sequence and protein structure databases. Because a major goal of the Genome Project is to map and sequence the genomes of several experimental animals, including E. coli, yeast, fruit fly, nematode, and mouse, the available databases for those organisms are listed as well. The author also includes several databases that are still under development - including some ambitious efforts that go beyond data compilation to create what are being called electronic research communities, enabling many users, rather than just one or a few curators, to add or edit the data and tag it as raw or confirmed.

Methods and Findings FDA Drug Approval Packages for eight second-generation antipsychoticsaripiprazole, iloperidone, olanzapine, paliperidone, quetiapine, risperidone, risperidone long-acting injection (risperidone LAI), and ziprasidonewere used to identify a cohort of 24 FDA-registered premarketing trials. The results of these trials according to the FDA were compared with the results conveyed in corresponding journal articles. The relationship between study outcome and publication status was examined, and effect sizes derived from the two data sources were compared. Among the 24 FDA-registered trials, four (17%) were unpublished. Of these, three failed to show that the study drug had a statistical advantage over placebo, and one showed the study drug was statistically inferior to the active comparator. Among the 20 published trials, the five that were not positive, according to the FDA, showed some evidence of outcome reporting bias. However, the association between trial outcome and publication status did not reach statistical significance. Further, the apparent increase in the effect size point estimate due to publication bias was modest (8%) and not statistically significant. On the other hand, the effect size for unpublished trials (0.23, 95% confidence interval 0.07 to 0.39) was less than half that for the published trials (0.47, 95% confidence interval 0.40 to 0.54), a difference that was significant. Conclusions The magnitude of publication bias found for antipsychotics was less than that found previously for antidepressants, possibly because antipsychotics demonstrate superiority to placebo more consistently. Without increased access to regulatory agency data, publication bias will continue to blur distinctions between effective and ineffective drugs.
