    De plus en plus de données probantes tirées d’essais cliniques et d’études épidémiologiques indiquent qu’une fréquence cardiaque élevée au repos est un prédicteur d’événements cliniques. La preuve de sa cause et de son effet direct est limitée car les médicaments courants qui réduisent la fréquence cardiaque (p. ex., les bêtabloquants) ont de multiples mécanismes d’action. Une nouvelle catégorie de médicament, les inhibiteurs If sélectifs, est en cours d’exploration à titre de « pur » médicament réduisant la fréquence cardiaque, ce qui aidera à confirmer s’il existe un lien fortuit entre la fréquence cardiaque élevée et les issues cardiovasculaires. Le présent article analyse les données probantes selon lesquelles une fréquence cardiaque élevée est un facteur de risque cardiovasculaire ainsi que certains essais cliniques courants qui évaluent cette hypothèse.

    Introduction Les maladies cardiovasculaires (MCV) constituent l'une des principales causes de mortalité dans les pays en développement. Le dépistage de ces dernières chez des jeunes est un défi dans la lutte contre leur expansion. Le but de cette étude était de dépister ces maladies au sein d'une population jeunes d’étudiants camerounais. Methodes Deux mille six cent cinquante-huit étudiants de l'Université de Douala (23,6 ± 2,9 ans, sex-ratio H/F = 0,9) ont en Avril - Mai 2011 participé à une campagne de dépistage gratuit du diabète, de l'hypertension artérielle (HTA) et de l'obésité. Ils ont également été soumis à une d'enquête évaluant leur niveau en activités physiques et sportives (APS). Resultats 12,7% des participants avaient une pression artérielle (PA) ≥ 140/90 mmHg, 3,6% étaient obèses et 0,9% avaient une glycémie ≥1,26 g/L. Des corrélations ont été trouvées entre certains facteurs de risque (diabète, hypertension et obésité) et le niveau académique d'une part (r =0,366; p < 0,0001) et le temps passé devant la télévision d'autres part (r = 0,411; p < 0,0001). L‘APS était inversement corrélée à l‘âge (r =-0,015; p < 0,0001) et au temps passé devant la télévision (r = -0,059; p = 0,002). Conclusion La présence des MCV et leurs facteurs de risque mis en évidence dans cette étude réalisée en milieu estudiantin camerounais interpelle à une prévention et une éducation dans la lutte contre ces dernières.   3. Morbidité et mortalité hospitalière des maladies cardiovasculaires en milieu tropical: exemple d'un centre hospitalier à Lomé (Togo)

    Introduction Les maladies cardiovasculaires (MCV) occupent une place importante dans la mortalité en Afrique, situation inquiétante traduisant une transition épidémiologique rapide. La connaissance des groupes nosologiques les plus mortelles devrait aider à l’élaboration de politiques en matière de traitement et de prévention. Méthodes Il's agit d une étude rétrospective du 1er janvier 2006 au 31 décembre 2010, ayant inclus les patients hospitalisés pour une MCV selon la classification Internationale des maladies (CIM-10). Résultats La fréquentation hospitalière pour maladie MCV a augmenté au fil du temps; les maladies hypertensives étaient le premier motif d hospitalisation (66.8%). Les autres pathologies fréquentes étaient la maladie thromboembolique (9.1%), les syndromes coronaires aigus (SCA) (7.3%), l'insuffisance cardiaque (5.5%), les cardiomyopathies (5.3%). La majorité des sujets étaient jeunes (âge moyen: 55.1 ans) et il n'y avait pas de corrélation entre la durée d'hospitalisation et l’âge: r = + 0.024, p = 0.09. La mortalité globale était élevée (11%) et les taux de létalité élevés dans le SCA (27.9%), l'embolie pulmonaire (25%) et la péricardite (25%). Conclusion Les MCV sont responsables d'une hospitalisation croissante dans nos hôpitaux. L'absence d unité de soins intensifs cardiologiques et des méthodes de revascularisation coronaire, le manque de personnels qualifiés associés l'inexistence de sécurité sociale expliquent une mortalité élevée dans nos pays en voie de développement qui sans avoir achevé leur transition démographique entre en pleine transition épidémiologique.   4. Facteurs de risque cardiovasculaires au cours du lupus systémique

    Cette étude a pour objectif d’évaluer la fréquence des facteurs de risque cardiovasculaires au cours du lupus et de préciser leur prévalence. Etude rétrospective portant sur 250 patients ayant un lupus, diagnostiqué selon les critères de l'ACR, hospitalisés entre 1970 et 2013. Les données cliniques et para cliniques ont été recueillies à partir des observations médicales. Il s'agit de 228 femmes et 22 hommes d’âge moyen au diagnostic du lupus de 30, 32 ans (extrêmes: 16-69). La durée moyenne du suivi des patients était de 64 mois (extrêmes: 7 jours- 382mois). Quatre vingt dix patients (36%) étaient hypertendus, 74% avaient une hypercholestérolémie et 22% étaient diabétiques. Pour les autres facteurs de risque cardiovasculaire traditionnels, un âge > 50 ans a été retrouvé dans 40% des cas, le sexe masculin dans 8% des cas, l'obésité dans 76% des cas et le tabagisme dans 11% des cas. Les facteurs de risque surajoutés sont représentés par la présence des anticorps antiphospholipides (47% des cas), la néphropathie lupique (49% des cas), l'insuffisance rénale (42% des cas), la corticothérapie au long cours (74% des cas) et la chronicité de la maladie dans 35% des cas. Les complications cardiovasculaires retrouvées dans notre série étaient: les accidents vasculaires cérébraux (2%) et l'insuffisance coronarienne (5,6%). Devant l'importance du risque cardiovasculaire au cours du lupus, une surveillance rapprochée des facteurs de risque cardio-vasculaires semble primordiale chez les lupiques. PMID:27022427

  5. Epaisseur de l'intima-média carotidienne et facteurs de risque cardio-vasculaires

    Introduction L’épaisseur intima-média de la carotide commune mesurée à l’échographie est un marqueur de risque cardio-vasculaire. L'objectif de ce travail est d’établir la corrélation entre l’épaisseur de l'intima-média carotidienne commune et les facteurs de risque cardio-vasculaire chez des sujets asymptomatiques. Méthodes Etude transversale descriptive et analytique portant sur 77 sujets de 40 ans et plus chez qui nous avons évalué les facteurs de risque cardio-vasculaire et analysé leur association avec l’épaisseur intima-média carotidienne commune. Résultats L’épaisseur intima-média augmentait avec l’âge. Les hommes avaient une épaisseur intima-média plus marquée que les femmes. L'hypertension artérielle, le diabète et la dyslipidémie sont corrélés à l’épaisseur de l'intima-média contrairement au tabagisme. Conclusion L’âge élevé et le sexe masculin sont les facteurs déterminants de la majoration de l’épaisseur intima-média carotidienne commune surtout si s'ajoutent l'hypertension artérielle, le diabète ou la dyslipidémie.     Introduction Dans le but de déterminer la fréquence des troubles du sommeil, de l'anxiété et de la dépression, et de rechercher l'existence d'une relation entre ces troubles et le risque cardio-vasculaire global chez les hypertendus noirs africains, nous avons réalisé une étude transversale de mai 2010 à mars 2011 à l'unité de consultation externe du service de cardiologie du CHU-Yalgado Ouédraogo de Ouagadougou qui a inclut 414 hypertendus adultes suivis en ambulatoire. Après un examen clinique à la recherche des facteurs de risque cardio-vasculaire, deux auto-questionnaires ont été administrés. Méthodes Le questionnaire de l’«European Sleep Center » pour la recherche des troubles du sommeil et, l’échelle «Hospital Anxiety and Depression Scale » de Zigmond et Snaith pour la recherche de l'anxiété et la dépression. Le diagnostic du syndrome d'apnée du sommeil était clinique et basé sur la présence des 4 symptômes principaux: hypersomnolence diurne, éveils nocturnes fréquents avec nycturie, asthénie matinale avec ou sans céphalées et ronflements importants. Le risque cardio-vasculaire global était calculé grâce à l’équation d'Anderson tirée de l’étude de Framingham. L'analyse des données a été réalisée par le logiciel SPSS version 17. La comparaison des variables a été effectuée grâce au test de Khi 2 pour les variables qualitatives et au test «t» de Student pour les variables quantitatives. Le seuil de signification a été fixé à 5%. Résultats L’échantillon était composé de 414 patients dont 248 femmes (59,9%). L’âge moyen était de 54,6 ± 9,3 ans. Les troubles du sommeil étaient retrouvés dans 72,2% des cas. Ils étaient dominés par l'insomnie (49,2%), le syndrome d'apnée du sommeil (33,5%) et le syndrome des jambes sans repos (25,8%).     Introduction L’évaluation cardiovasculaire est essentielle en hémodialyse périodique car les affections cardiovasculaires sont la première cause de mortalité chez les hémodialysés chroniques. Nous avons conduit cette étude afin de déterminer la prévalence et le type des différentes complications cardiovasculaires et d'identifier les principaux facteurs de risque cardiovasculaire. Méthodes Il s'agissait d'une étude rétrospective de 4 ans portant sur les dossiers de patients traités au moins 6 mois en hémodialyse et ayant des explorations cardio-vasculaires comportant une radiographie du thorax de face, un électrocardiogramme et une échographie cardiaque. Les données épidémiologiques, cliniques, paracliniques, les aspects évolutifs des complications cardiaques ont été recueillies pour chaque patient retenu. Résultats Trente huit dossiers de patients ont été inclus dans cette étude. L’âge moyen était de 52 ans ± 12,85 et le sex-ratio H/F de 1,53. Les complications cardiovasculaires étaient dominées par l'hypertrophie ventriculaire gauche (71,05%), la maladie coronaire (34,21%), l'insuffisance cardiaque congestive (18,42%), Calcifications vasculaires (5,78%), les troubles du rythme (7.89%), les fuites valvulaires mitrales (44,73%), tricuspidiennes (42,10%) et les accidents vasculaires cérébraux hémorragiques (5,26%). L'incidence moyenne de l'HVG était de 81% chez les patients hypertendus. Au cours de cette étude, 27 patients avaient poursuivi l'hémodialyse et 11 étaient décédés dont 6 de causes cardiovasculaires (54,55%). Conclusion L'hémodialyse est une technique d’épuration qui expose les patients à de multiples complications cardiovasculaires. PMID:27217869

  8. L'obésité en consultation cardiologique à Lomé: prévalence et facteurs de risque cardio-vasculaire associés - étude chez 1200 patients

    Introduction Introduction: Les objectifs de ce travail étaient de déterminer la fréquence de l'obésité et celle des autres facteurs de risque cardio-vasculaire chez ces patients obèses à Lomé (Togo). Méthodes Il s'est agi d'une étude multicentrique transversale de prévalence. Elle s'est déroulée du 05 septembre 2005 au 04 mars 2006 et a porté sur les malades vus en consultation externe dans 3 services de cardiologie de la commune de Lomé. Ont été inclus dans cette étude les malades ayant un surpoids selon les normes de l'OMS. Résultats Parmi 1200 patients vus en consultations, 779 (64,92%) avaient une surcharge pondérale. L’âge moyen était de 49,53 ± 17,24 ans. L'obésité était plus fréquente chez les femmes (79,49%) que chez les hommes (20,51%). Un antécédent d'obésité familiale (61,8%) était le principal facteur favorisant. Les autres facteurs de risque cardio-vasculaire retrouvés étaient: sédentarité (82% vs50% chez les non obèses), hypertension artérielle (54,8% vs 39,2%), alcool (50,9% vs 43,9%), dyslipidémie (34,5% vs 20%), diabète (30,9% vs 10,7%) et tabac (14,1% vs 20,3%). La différence était statistiquement significative entre les deux groupes. Les principales complications cardiovasculaires observées chez les obèses étaient: l'ischémie myocardique (26,7%), l'hypertrophie ventriculaire gauche (46,4%), la dilatation cavitaire cardiaque (30,1%) et les accidents vasculaires cérébraux (7,1%). Conclusion L'obésité est un problème de santé publique au Togo. Sa prévalence est très élevée et elle est le plus souvent associée aux autres facteurs de risque cardio-vasculaire.     Résumé Objectif Comprendre comment les nouvelles lignes directrices sur la prise en charge du risque cardiovasculaire publiées par l’American Heart Association et l’American College of Cardiology (AHA-ACC) peuvent être interprétées et utilisées dans le contexte canadien. Sources des données Les lignes directrices de l’AHA-ACC ont été examinées, de même que toutes les références. Des recherches indépendantes dans PubMed ont été effectuées pour inclure l’ajout aux statines d’autres antihyperlipidémiants et l’emploi de calculatrices médicales visant à aider le patient à comprendre. Message principal Les nouvelles lignes directrices de l’AHA-ACC s’appuient sur les meilleures données probantes disponibles actuellement en matière de traitement de la dyslipidémie. Cela inclut le risque cardiovasculaire (CV) sur 10 ans comme seuil thérapeutique au lieu du taux de cholestérol des lipoprotéines de faible densité, de même que l’abandon des cibles thérapeutiques de lipoprotéines de faible densité. On met de plus en plus l’accent sur les interventions alimentaires et l’exercice, avec l’amorce d’un effort pour quantifier l’effet de ces interventions. Les statines constituent la principale intervention pharmacologique, et l’ajout d’autres médicaments pour accentuer la baisse lipidique n’est plus recommandé. En ce qui concerne l’application au Canada, les tables de risque de Framingham sont plus appropriées pour évaluer le risque que les équations de cohortes regroupées utilisées aux États-Unis. Les calculatrices du risque de maladie cardiovasculaire (MCV) devraient contenir de l’information sur l’âge cardiovasculaire et devraient pouvoir illustrer le risque et les interventions de rechange afin d’aider le patient à comprendre et de favoriser la prise de décision éclairée.     PurposeThin-strut, flexible DES are suitable for intracranial stenting and may improve long-term vessel patency. The purpose of this study was to report our experience with two new-generation DES for the treatment of intracranial atherosclerotic disease.Patients and MethodsWe retrospectively reviewed all patients treated with Taxus Element™ or Resolute Integrity™ stent for an intracranial atherosclerotic stenosis between March 2011 and August 2013. Technical success was defined as the ability to deploy the device at the desired location and reduce the degree of stenosis below 50 %. All procedure-related strokes were recorded. Control angiography was scheduled after 6 weeks, 3, 6, and 12 months and yearly thereafter. A luminal narrowing ≥50 % was regarded as a restenosis. Stroke recurrence in the territory of the treated artery was reported.ResultsIn the defined period we treated 101 patients harboring 117 intracranial stenoses. The procedure was successful in 100 (85.5 %) lesions. Procedure-related strokes occurred in 10 (9.9 %) patients with a permanent morbidity in 3 (3.0 %). Follow-up angiography was available for 83 of 100 (83.0 %) successfully treated lesions with an average of 355 days (IQR 153–482 days). Three (3.6 %) asymptomatic restenoses were detected. All occurred after treatment with Resolute Integrity™, none after Taxus Element™ (p = 0.059). New strokes in the territory of the treated artery were encountered in 2 (2.6 %) occasions.ConclusionThin-strut DES improve long-term patency and reduce the risk of subsequent stroke. Differences between devices may exist. Feasibility is comparable to bare-metal balloon-mounted stents and procedure-related strokes occurred within the expected range.

  11. Approche à l’égard des nouveaux anticoagulants oraux en pratique familiale

    Résumé Objectif Comparer les caractéristiques principales des nouveaux anticoagulants oraux (NACO), soit le dabigatran, le rivaroxaban et l’apixaban, et répondre aux questions qui font surface lors de la comparaison de ces agents. Qualité des données Une recherche dans PubMed a été effectuée afin de relever les études cliniques récentes (de janvier 2008 à la semaine 32 de 2013) portant sur l’emploi des NACO pour la prévention des AVC dans les cas de fibrillation auriculaire (FA) et pour le traitement de la thromboembolie veineuse aiguë. Message principal Selon 3 essais d’envergure, tous les NACO sont au moins aussi efficaces que la warfarine dans la prévention des AVC chez les patients atteints d’une FA non valvulaire, et au moins aussi sûrs pour ce qui est du risque de saignement. Des méta-analyses de ces essais ont montré que, comparativement au traitement par la warfarine, les NACO avaient réduit la mortalité totale, la mortalité d’origine cardiovasculaire et les saignements intracrâniens, et était aussi ressortie une tendance vers la réduction des saignements généraux. Du côté pratique, les avantages des NACO par rapport à la warfarine sont : posologie orale fixe uniquotidienne ou biquotidienne sans devoir surveiller la coagulation et peu d’interactions connues ou définies avec d’autres médicaments ou des aliments. Les désavantages potentiels des NACO sont notamment un risque de saignement qui serait accru chez les patients de plus de 75 ans, une hausse des saignements gastro-intestinaux majeurs avec des doses élevées de dabigatran, une hausse des cas de dyspepsie avec le dabigatran, l’absence d’un test de laboratoire de routine visant à mesurer de façon fiable l’effet anticoagulant et l’absence d’antidote pour renverser l’effet anticoagulant.   14. Apport de l'orientation clinique dans le diagnostic des hypertensions arterielles endocriniennes

    L'hypertension artérielle d'origine endocrinienne représente une cause curable d'hypertension artérielle (HTA), ce qui en justifie le dépistage. Une orientation clinique est souhaitable afin de prescrire les bilans adéquats, notamment dans les pays en voie de développement. L'objectif de notre étude est décrire les aspects cliniques des patients ayant une HTA endocrinienne diagnostiquée au CHU de Casablanca. Méthodes: Il s'agit d'une étude descriptive rétrospective de 2004 à 2011, incluant tout patient ayant une HTA endocrinienne (en dehors des dysthyroïdies) admis en Endocrinologie. Les données concernant les facteurs cliniques, paracliniques et thérapeutiques ont été colligées. Au cours de la période étudiée, 53 cas d'HTA endocrinienne ont été admis, avec une age moyen au diagnostic de 39,3 ans et un sex-ratio F/H de 3,9. Trente patients (56.6%) ont un age ont un âge inférieur à 40 ans. On ne retrouve pas d'hérédité tensionelle chez 88.7% des patients. Les étiologies sont le Syndrome de cushing (41.5%), le phéochromocytome (32%), les adénomes somatotropes (15%) et les hyperaldostéronismes primaire (7.5%). L'HTA est essentiellement de grade un ou deux (69.8%), avec un retentissement cardio-vasculaire chez 11 patients (20.7%). Les patients ayant un SC avaient tous des signes cutanés spécifiques. Il s'agissait de sujets jeunes avec plusieurs facteurs de risque cardiovasculaire. Nous avons retrouvé un amaigrissement chez 37.5% des patients ayant un phéochromocytome avec un indice de masse corporel moyen de 20 kg/m2. L'HTA était permanente dans 5 cas de phéochromocytomes. Il n'existait pas d'hypokaliémie chez 50% des patients ayant un hyperaldostéronisme. Les signes cliniques des patients ayant une acromégalie étaient francs, avec un age moyen plus élevé de 50 ans.     Introduction L'objectif de notre étude était de décrire les caractéristiques sociodémographiques et les Facteurs de Risque cardio-Vasculaires (FRV) des patients admis pour accidents vasculaires cérébraux (AVC) dans un service autre que celui de la neurologie. Méthodes Étude transversale rétrospective sur une période de 2 ans (janv. 2010 et déc. 2011), réalisée aux urgences de l'institut de cardiologie d'Abidjan. Résultats Il s'agissait de 176 adultes avec un âge moyen de 60 ans, une prédominance féminine. Les facteurs de risque majeurs retrouvés étaient l'hypertension artérielle dans 86,4% des cas, le diabète dans 11,4% des cas, le tabagisme dans 2,2% des cas. Les motifs de consultation étaient la perte de connaissance dans 36,4% des cas, l'hémiplégie dans 31,8% des cas, les céphalées dans 17,4% des cas, les vertiges dans 10,9% et les palpitations dans 2,2% des cas. La tension artérielle systolique moyenne était à 174 mmHg, la tension artérielle diastolique moyenne était à 105 mmHg et la pression pulsée moyenne était à 70 mmHg. Les AVC étaient associés à une arythmie complète par fibrillation auriculaire dans 11,4% des cas. Les AVC ischémiques représentaient 84,1%. L’évolution aux urgences a été marquée par un décès dans 17% (30) des cas. Conclusion Les AVC constituent un problème majeur de santé publique. Malgré sa prédominance féminine, ils (AVC) touchaient 44% des hommes dans notre étude lorsqu'on sait qu'en Afrique l'activité sociale repose sur les hommes. Ils restent une pathologie grave par la forte létalité. PMID:26327997

  17. Etude exploratoire des conceptions de la circulation sanguine aupres d'eleves de l'ordre collegial

Ces reponses indiquent egalement que la dynamique circulatoire est reduite a une sequence d'etapes ponctuelles sans relation les unes avec les autres. Au troisieme niveau les reponses des eleves de la population font etat d'une conception du controle qui privilegie la satisfaction de besoins locaux, sans relation systemique. Nos resultats suggerent que les eleves de notre echantillon affichent une plus grande concordance avec l'expert pour les questions du premier niveau (70%) que pour les niveaux II (54%) et III (50%). Notre analyse des donnees revele que l'accord avec l'expert est eleve lorsque la questions touchent la description des structures et la definition de leurs roles et plus faible lorsque les questions touchent la dynamique et le controle. Il existerait donc un niveau de formulation qui correspond a la description de structures et un autre niveau qui recoupe toute la dynamique de la circulation et son controle. Du point de vue didactique lanalyse des donnees suggere que nous ne retrouvons pas une correspondance entre les niveaux de formulation didactique et les conceptions des eleves.   18. Restauration fonctionnelle du rachis : effet du niveau initial de douleur sur les performances des sujets lombalgiques chroniques

    HISTORIQUE : La lombalgie chronique est une douleur lombaire persistante d’origine multifactorielle. Le niveau de douleur initial reste faiblement utilisé pour analyser et comparer les réponses des patients lombalgiques au programme de reconditionnement. OBJECTIFS : Apprécier et évaluer les réponses des sujets lombalgiques chroniques très douloureux à une prise en charge dynamique et intensive. MÉTHODOLOGIE : 144 sujets atteints de lombalgie chronique ont été inclus dans un programme de restauration fonctionnelle du rachis de 5 semaines. Les sujets ont été classés en deux groupes de niveau de douleur: un groupe atteint de douleur sévère (n = 28) et un groupe atteint de douleur légère à modérée (n = 106). L’ensemble des sujets ont bénéficié d’une prise en charge identique comprenant principalement de la kinésithérapie, de l’ergothérapie, du reconditionnement musculaire et cardio-vasculaire ainsi qu’un suivi psychologique. Les paramètres physiques (flexibilité, force musculaire) et psychologiques (qualité de vie) ont été mesurés avant (T0) et après le programme (T5sem). RÉSULTATS : L’ensemble des performances physiques et fonctionnelles des sujets très douloureux sont moins bonnes et le retentissement de la lombalgie sur la qualité de vie, pour ces mêmes sujets, est majoré à T0. Toutes les différences significatives constatées à T0 entre les deux groupes s’effacent à T5sem. CONCLUSIONS : Les sujets lombalgiques chroniques très douloureux répondent favorablement au programme dynamique et intensif. L’intensité douloureuse de la lombalgie n’aurait pas d’effet sur les réponses au programme. La restauration fonctionnelle du rachis apporterait aux sujets la possibilité de mieux gérer leur douleur quel que soit son niveau. PMID:25299476

    A partir d'une théorie sur la détermination des formes et des contraintes globales d'un ballon de révolution, ou s'en rapprochant, une nouvelle famille de ballons a été définie. Les ballons actuels, dits de ``forme naturelle'', sont calculés en général pour une tension circonférencielle nulle. Ainsi, pour une mission donnée, la tension longitudinale et la forme de l'enveloppe sont strictement imposées. Les ballons de la nouvelle génération sont globalement cylindriques et leurs pôles sont réunis par un câble axial, chargé de transmettre une partie des efforts depuis le crochet (pôle inférieur), directement au pôle supérieur. De plus, la zone latérale cylindrique est soumise à un faible champ de tensions circonférencielles. Ainsi, deux paramètres permettent de faire évoluer la distribution des tensions et la forme de l'enveloppe: - la tension du câble de liaison entre pôles (ou la longueur de ce câble) - la tension circonférencielle moyenne désirée (ou le rayon du ballon). On peut donc calculer et réaliser: - soit des ballons de forme adaptée, comme les ballons à fond plat pour le bon fonctionnement des montgolfières infrarouge (projet MIR); - soit des ballons optimisés pour une bonne répartition des contraintes et une meilleure utilisation des matériaux d'enveloppe, pour l'ensemble des programmes stratosphériques. Il s'ensuit une économie sensible des coûts de fabrication, une fiabilité accrue du fonctionnement de ces ballons et une rendement opérationnel bien supérieur, permettant entre autres, d'envisager des vols à très haute altitude en matériaux très légers.

    Les auteurs présentent les cas d'amputation des quatre membres réalisée chez trois patients différents. Ce sont des amputations réalisées pour chaque patient au cours d'une seule hospitalisation et en un seul temps opératoire. Deux patients pour gangrène sèche infectée et un pour amputation traumatique des quatre membres. Cette zoologie diversifiée fait pourtant l'objet d'un commun effort de modélisation. Le but de cette revue est d'abord de présenter qualitativement le développement, depuis leur origine, des diverses théories de vents (Partie 1) et l'inter disciplinarité dans ce domaine. Il s'agit d'une énumération, plus ou moins exhaustive, des idées proposées pour expliquer l'accélération et la morphologie des vents et des jets, accompagnée d'une présentation sommaire des aspects observationnels. Cette partie s'abstient de tout aspect faisant appel au formalisme mathématique. Ces écoulements peuvent être décrits, au moins partiellement, en résolvant les équations magnétohydrodynamiques, axisymétriques et stationnaires.     Restenosis after percutaneous coronary interventions has been a major limitation of this otherwise very well-accepted method of coronary revascularisation. Coronary stents work by scaffolding the intimal flaps and preventing elastic recoil, which was a major problem after balloon angioplasty. The neointimal growth response to stenting contributes significantly to the restenotic process. Randomised studies comparing coronary artery bypass surgery with coronary stenting especially in multivessel disease clearly highlighted this problem. The problem has been greater in magnitude in special subgroups: diabetics, patients with small vessels (≤2.5 mm in diameter), long segments of disease (≥20 mm in length), etc. These limitations of Bare metal stents have been addressed by drug-eluting stents (DESs). Third-generation stents with bioabsorbable polymers like the Biolimus releasing Biomatrix stent have already become available in Europe and parts of Asia. A longer follow-up will prove their long-term safety vis-à-vis first-generation DES. The polymer-free stent with capability of using more than one drug, though very attractive, needs larger multicentric studies before gaining wider acceptance. The fully bioabsorbable stent is yet another promising concept. The feasibility has already been demonstrated, and finer refinements are under way. The future of newer DES thus is very promising, and most of the issues related to first-generation DES are at the threshold of being solved.

Dermatologists should nurture strong physician-patient relationships with proper informed consent and stay abreast of legal issues as they pertain to today's practice of medicine. Medicolegal issues that have risen to the forefront include wrong-site surgery, delegation of procedures to nonphysician operators, and compounding of medications. Additionally, although the marriage of health care and technology has facilitated our practice of medicine, it has opened doors to new medicolegal pitfalls associated with the use of electronic medical records, teledermatology, and even social media.   8. Reticulation des fibres lignocellulosiques

    Pour faire face à la crise économique la conception de papier à valeur ajoutée est développée par les industries papetières. Le but de se projet est l'amélioration des techniques actuelles de réticulation des fibres lignocellulosiques de la pâte à papier visant à produire un papier plus résistant. En effet, lors des réactions de réticulation traditionnelles, de nombreuses liaisons intra-fibres se forment ce qui affecte négativement l'amélioration anticipée des propriétés physiques du papier ou du matériau produit. Pour éviter la formation de ces liaisons intra-fibres, un greffage sur les fibres de groupements ne pouvant pas réagir entre eux est nécessaire. La réticulation des fibres par une réaction de « click chemistry » appelée cycloaddition de Huisgen entre un azide et un alcyne vrai, catalysée par du cuivre (CuAAC) a été l'une des solutions trouvée pour remédier à ce problème. De plus, une adaptation de cette réaction en milieux aqueux pourrait favoriser son utilisation en milieu industriel. L'étude que nous désirons entreprendre lors de ce projet vise à optimiser la réaction de CuAAC et les réactions intermédiaires (propargylation, tosylation et azidation) sur la pâte kraft, en milieu aqueux. Pour cela, les réactions ont été adaptées en milieu aqueux sur la cellulose microcristalline afin de vérifier sa faisabilité, puis transférée à la pâte kraft et l'influence de différents paramètres comme le temps de réaction ou la quantité de réactifs utilisée a été étudiée. Dans un second temps, une étude des différentes propriétés conférées au papier par les réactions a été réalisée à partir d'une série de tests papetiers optiques et physiques. To launch its product, she was associated to competency characters (engineer, pharmacist, physician) and benefited from the protection of the imperial family. A lawsuit between the associates and the 1870 war hindered the company's early days, but "L'Eau des Fées" provided with quality pledges, imposed itself definitely. The "Parfumerie des Fées" completed the range of its cosmetics with creams, powders, etc. Sarah Félix paid attention to present "L'Eau des Fées" as a dye devoid of toxicity.     Résumé Objectif Définir la pratique de la médecine des voyages, présenter les éléments fondamentaux d’une consultation complète préalable aux voyages à des voyageurs internationaux et aider à identifier les patients qu’il vaudrait mieux envoyer en consultation auprès de professionnels de la médecine des voyages. Sources des données Les lignes directrices et les recommandations sur la médecine des voyages et les maladies liées aux voyages publiées par les autorités sanitaires nationales et internationales ont fait l’objet d’un examen. Une recension des ouvrages connexes dans MEDLINE et EMBASE a aussi été effectuée. Message principal La médecine des voyages est une spécialité très dynamique qui se concentre sur les soins préventifs avant un voyage. Une évaluation exhaustive du risque pour chaque voyageur est essentielle pour mesurer avec exactitude les risques particuliers au voyageur, à son itinéraire et à sa destination et pour offrir des conseils sur les interventions les plus appropriées en gestion du risque afin de promouvoir la santé et prévenir les problèmes médicaux indésirables durant le voyage. Des vaccins peuvent aussi être nécessaires et doivent être personnalisés en fonction des antécédents d’immunisation du voyageur, de son itinéraire et du temps qu’il reste avant son départ. Conclusion La santé et la sécurité d’un voyageur dépendent du degré d’expertise du médecin qui offre le counseling préalable à son voyage et les vaccins, au besoin. On recommande à ceux qui donnent des conseils aux voyageurs d’être conscients de l’ampleur de cette responsabilité et de demander si possible une consultation auprès de professionnels de la médecine des voyages pour tous les voyageurs à risque élevé.

    The declining pool of graduates, the lack of rigorous preparation in science and mathematics, and the declining interest in science and engineering careers at the precollege level promises a shortage of technically educated personnel at the college level for industry, government, and the universities in the next several decades. The educational process, which starts out with a large number of students at the elementary level, but with an ever smaller number preparing for science and engineering at each more advanced educational level, is in a state of crisis. These pipeline issues, so called because the educational process is likened to a series of ever smaller constrictions in a pipe, were examined in a workshop at the Space Grant Conference and a summary of the presentations and the results of the discussion, and the conclusions of the workshop participants are reported.

    Hybrid RANS/LES methods are being increasingly used for turbulent flow simulations in complex geometries. Spalart's detached eddy simulation (DES) model is one of the more popular ones. We are interested in examining the behavior of the Spalart-Allmaras (S-A) Detached Eddy Simulation (DES) model in its ``LES mode.'' The role of the near-wall functions present in the equations is analyzed and an explicit analogy between the S-A and a one-equation LES model based on the sub-grid kinetic energy is presented. A dynamic version of the S-A DES model is proposed based on this connection. Validation studies and results from DES and LES applications will be presented and the effect of the proposed modification will be discussed.

    Peste des petits ruminants virus causes a highly infectious disease of small ruminants that is endemic across Africa, the Middle East and large regions of Asia. The virus is considered to be a major obstacle to the development of sustainable agriculture across the developing world and has recently been targeted by the World Organisation for Animal Health (OIE) and the Food and Agriculture Organisation (FAO) for eradication with the aim of global elimination of the disease by 2030. Fundamentally, the vaccines required to successfully achieve this goal are currently available, but the availability of novel vaccine preparations to also fulfill the requisite for differentiation between infected and vaccinated animals (DIVA) may reduce the time taken and the financial costs of serological surveillance in the later stages of any eradication campaign. Here, we overview what is currently known about the virus, with reference to its origin, updated global circulation, molecular evolution, diagnostic tools and vaccines currently available to combat the disease. Further, we comment on recent developments in our knowledge of various recombinant vaccines and on the potential for the development of novel multivalent vaccines for small ruminants. PMID:26443889

    DES exposure checker renders science-grade images directly to a web browser and allows users to mark problematic features from a set of predefined classes, thus allowing image quality control for the Dark Energy Survey to be crowdsourced through its web application. Users can also generate custom labels to help identify previously unknown problem classes; generated reports are fed back to hardware and software experts to help mitigate and eliminate recognized issues. These problem reports allow rapid correction of artifacts that otherwise may be too subtle or infrequent to be recognized.

    structure des oscillations de conductance en fonction du flux du champ magnétique de période h/e dont l'amplitude est beaucoup plus importante que celle mesurée sur un réseau carré de même dimension. Cette différence constitue une signature d'un effet de localisation induit par le champ magnétique sur la topologie mathcal{T}3. Pour des valeurs spécifiques du champ magnétique, du fait des interférences destructives Aharonov-Bohm, la propagation des fonctions d'ondes est limitée à un ensemble fini de cellule du réseau appelé cage. De la dépendance en température des oscillations de période h/e mesurées sur le réseau mathcal{T}3 nous avons tiré une longueur caractéristique qui peut être rattachée au périmètre des cages. Un phénomène inattendu fut l'observation, pour des champs magnétiques plus importants, d'un doublement de fréquence des oscillations. Ces oscillations de période h/2e pouvant avoir une amplitude supérieure aux oscillations de période h/e, une interprétation en terme d'harmonique n'est pas possible. Enfin, l'influence de la largeur électrique des fils constituant le réseau et donc celle du nombre de canaux par brin a été étudiée en réalisant des grilles électrostatique. Les variations de l'amplitude des signaux en h/e et h/2e en fonction de la tension de grille ont été mesurés.

    L'assemblage par soudage peut engendrer des contraintes residuelles. Ces contraintes provoquent des fissurations prematurees et un raccourcissement de la duree de vie des composants. Dans ce contexte, le martelage robotise est utilise pour relaxer ces contraintes residuelles. Trois volets sont presentes: le premier est l'evaluation des effets des impacts unitaires repetes sur le champ de contraintes developpe dans des plaques d'acier inoxydable austenitique 304L vierges ou contenant des contraintes residuelles initiales. Dans la deuxieme partie de ce projet, le martelage est applique grace au robot SCOMPI. Les contraintes residuelles induites et relaxees par martelage sont ensuite mesurees par la methode des contours, qui a ete adaptee a cet effet. Dans la troisieme partie, le martelage est modelise par la methode des elements finis. Un modele axisymetrique developpe grace au logiciel ANSYS permet de simuler des impacts repetes d'un marteau elastique sur une plaque ayant un comportement elastoplastique.

    Peste des petits ruminants virus (PPRV) causes a severe contagious disease of sheep and goats and has spread extensively through the developing world. Because of its disproportionately large impact on the livelihoods of low-income livestock keepers, and the availability of effective vaccines and good diagnostics, the virus is being targeted for global control and eventual eradication. In this review we examine the origin of the virus and its current distribution, and the factors that have led international organizations to conclude that it is eradicable. We also review recent progress in the molecular and cellular biology of the virus and consider areas where further research is required to support the efforts being made by national, regional, and international bodies to tackle this growing threat.     This study pertains to the process of Data Encryption Standard, DES. DES consists of encryption and decryption processes linked with mathematical elements such as algebra and number theory. Preliminary, studies revealed that most of mathematics students face a problem in understanding the complicated process of DES. In modern learning methods, learning environment becomes more interesting with the use of computer and a variety of mathematical software packages. Several mathematical softwares such as Maple, Mathematica, Mathlab and Sage were developed in order to fulfill the specific calculation requirements. Correspondingly, motivated from that, this study incorporated with Maple to enhance the existing DES Maplet interface to be more interactive and user-friendly compared to the original version.

    The Lawrence Berkeley National Laboratory (LBNL) Issues Management Program encompasses the continuous monitoring of work programs, performance and safety to promptly identify issues to determine their risk and significance, their causes, and to identify and effectively implement corrective actions to ensure successful resolution and prevent the same or similar problems from occurring. This document describes the LBNL Issues Management Program and prescribes the process for issues identification, tracking, resolution, closure, validation, and effectiveness of corrective actions. Issues that are governed by this program include program and performance deficiencies or nonconformances that may be identified through employee discovery, internal or external oversight assessment findings, suggested process improvements and associated actions that require formal corrective action. Issues may also be identified in and/or may result in Root Cause Analysis (RCA) reports, Price Anderson Amendment Act (PAAA) reports, Occurrence Reporting and Processing System (ORPS) reports, Accident Investigation reports, assessment reports, and External Oversight reports. The scope of these issues may include issues of both high and low significance as well as adverse conditions that meet the reporting requirements of the University of California (UC) Assurance Plan for LBNL or other reporting entities (e.g., U.S. Environmental Protection Agency, U.S. Department of Energy). Issues that are found as a result of a walk-around or workspace inspection that can be immediately corrected or fixed are exempt from the requirements of this document.

    Nous presentons dans cette these des modeles d'evolution stellaire incorporant la diffusion microscopique de maniere consistante. Pour la premiere fois, on a calcule l'evolution d'etoiles en tenant compte en detail de l'impact des variations d'abondances sur leur structure. Nous utilisons des spectres monochromatiques pour chacun des elements les plus abondants dans un melange solaire pour recalculer l'opacite pour les abondances et les conditions locales dans l'interieur d'une etoile au cours de son evolution. Nos modeles montrent que la diffusion atomique des metaux a un effet important sur les opacites dan les etoiles de plus de 1.3Msolar ou l'abondance du fer et des autres elements du pic du fer varient substantiellement. Ces etoiles, sans rotation ou champ magnetique, sont proches des etoiles de type Fm-Am dans lesquelles on observe une legere surabondance d'elements du pic du fer en plus d'une sous-abondance de calcium, sous-abondance que l'on obtient egalement. Nous obtenons cependant des surabondances depassant un facteur 10 pour les etoiles de plus de 1.4Msolar ce qui suggere qu'il existe un ou plusieurs mecanismes limitant la diffusion microscopique. La surabondance du fer en surface cause une augmentation, qui peut atteindre un facteur sept, de l'opacite a la limite de la zone convective. Ceci cause un accroissement de la temperature effective et de la masse de la zone convective comparativement aux modeles n'incluant que la diffusion de l'helium. Il s'agit la du principal effet de la diffusion sur la structure interne de ces etoiles. La diffusions n'a pas d'influence sur l'evolution de coeur stellaire dans les etoiles significativement plus massives quie le Soleil. Nous avons verife que l'utilisation de modeles consistants avec diffusion n'apporte pas d'amelioration sensible aux modeles solaires. Les forces radiatives calculees a partir des spectres d'OPAL pour les elements du pic du fer representent une fraction importante de la gravite. On obtient des

A review of such issues that have occurred over the last 50 years reveals that many of them have erupted rather suddenly (e.g., stratospheric ozone depletion, acid rain). However, some issues were recognized long ago by the scientific community (e.g., land degradation, overconsumption of freshwater), but for economic or other reasons governments have refused to act. The authors of this study were commissioned by the United Nations Environment Program and the Scientific Committee on Problems of the Environment to carry out a global survey of emerging environmental issues, using the responses received to questionnaires that were sent to scientists, managers, and policy makers around the world. It had been hoped that a short list of priority issues could be identified but the number of issues was very long. However, the issues could be divided into four major classes: 1) Transformations of old issues; continually evolving, and in most cases broadening, in response to increasing scientific and technological knowledge and to changing socioeconomic, cultural, and environmental conditions. 2) Policy issues, the long-term environmental consequences of which may already be of concern. 3) Accidents waiting to happen, for example, chemical time bombs. 4) Surprises in the nonlinear responses of ecosystems to new and different stresses, as well as in the nature of socioeconomic drivers of environmental change. In a subsequent study, the authors applied the lessons learned in the global study to an examination of emerging environmental issues in the province of Ontario, Canada.

    Increases in ES&H compliance issues within the past few years have necessitated a formal process by which DOE facilities address these issues. In May 1991, ANL-W implemented the ANL-W Issues Management System (IMS) to facilitate the management of compliance issues and scheduling of corrective action plans with limited resources. The central focus of this process is a computer database, Integrated Resource Management System (IRMS), which allows quick retrieval of compliance information, organization of compliance issues based on a risk-based prioritization methodology, and tracking of corrective action plans. Without the IRMS, the ANL-W Issues Management System would have been difficult to administer and manage. ANL-W has used the IRMS for both audit preparation and audit response, most noticeably the preparation and subsequent response to the 1991 Tiger Team audit. The IRMS was used to track ANL-W Self-Assessment corrective action plans, provide instant information to Tiger Team members regarding Self-Assessment findings, produce prioritized lists of Tiger Team concerns for developing corrective action plans, and track Tiger Team corrective action plans. Status reports to senior, laboratory management regarding the Tiger Team corrective action plan are produced based on information provided by the IRMS. This paper discusses the criteria used for selecting the IRMS, implementation of the Issues Management System using the IRMS, lessons learned, and the future evolution of the IRMS.

    Discussions about current and future immunisation programmes raise novel questions about familiar ethical issues. Two sets of ethical issues dominate these discussions. The first is the issue of compulsory immunisation: what should be done about parents who fail to immunise their children? The second is: given competing demands on health care budgets, how should principles of justice in access and distribution inform vaccination programmes? This paper considers these two issues in the light of traditional ethical principles. With respect to the first, we argue that compulsion is justified only in cases in which we know with practical certainty that parental failure to immunise puts their own child or other children at high risk of severe illness. We also argue that the state should compensate those who suffer vaccine-related injury. With respect to the second, we claim that allocating resources according to health care need requires establishing priorities between public health programmes such as immunisation and other treatment programmes.     In the framework of the GigaBit Transceiver project (GBT), a prototype, the GBT-SerDes ASIC, was developed, fabricated and tested. To sustain high radiation doses while operating at 4.8Gb/s, the ASIC was fabricated in a commercial 130 nm CMOS technology employing radiation tolerant techniques and circuits. The transceiver serializes-deserializes the data, Reed-Solomon encodes and decodes the data and scrambles and descrambles the data for transmission over optical fibre links. This paper describes the GBT-SerDes architecture, and presents the test results.

    Rail transportation is a vital segment of DOE's spent fuel program. Most of the shipments from reactors to a repository or to a Monitored Retrievable Storage facility will be by rail, and essentially all of the shipments from an MRS to a repository. Casks must move economically and efficiently, while at the same time providing adequate safety to the public and transport workers, and meeting the legal/institutional constraints. Shippers are faced with the problem of procuring transportation services in a safe manner at a reasonable cost, trying to balance freight charges against cask inventory costs. Carriers are cautious about accepting potentially high hazard materials in routine service, and are worried about uninsured losses in case of accidents. Both shippers and carriers operate under different scheduling and operational criteria. Technical and institutional constraints add to the complexity of the operation. Standardization of shipping casks and rail car design will ease the resolution of the complex problems now existent. This paper discusses the economic, safety, operational, and legal issues surrounding rail transportation.

    This paper considers the ethical issues raised by xenotransplantation under four headings: interfering with nature; effects on the recipient; effects on other humans; and effects on donor animals. The first two issues raise no insuperable problems: charges of unnaturalness are misguided, and the risks that xenotransplantation carries for the recipient are a matter for properly informed consent. The other two issues raise more serious problems, however, and it is argued that if we take seriously the risk of transferring new infectious agents from animal to human populations and the interests of donor animals, then a moratorium on xenotransplantation is called for. The paper finds that the recent Nuffield Council and Department of Health reports on xenotransplantation are insufficiently cautious in the conclusions that they draw from these considerations.     Au cours des dernières décennies, les infrastructures canadiennes et québécoises comportent plusieurs structures en béton armé présentant des problèmes de durabilité dus aux conditions climatiques sévères, à la mauvaise conception des structures, à la qualité des matériaux, aux types des bétons choisis, aux systèmes de construction ou à l'existence d'événements incontrôlables. En ce qui concerne le choix du béton pour la construction des infrastructures, une vaste gamme de béton divisée en deux principaux types peut être utilisée: le béton conventionnel vibré (BCV) et le béton autoplaçant (BAP). Dans le cas d'un BCV, la consolidation inadéquate par vibration a été un problème récurrent, occasionnant des dommages structuraux. Ceci a conduit à une réduction de la durabilité et à une augmentation du coût d'entretien et de réparation des infrastructures. Rien que l'utilisation d'un BAP a des avantages tels que l'élimination de la vibration, la réduction des coûts de main d'oeuvre et l'amélioration de la qualité des structures, néanmoins, le coût initial d'un BAP par rapport à un BCV ne permet pas encore de généraliser son utilisation dans l'industrie de la construction. Ce mémoire présente la conception d'une nouvelle gamme de béton semi-autoplaçant pour la construction des infrastructures (BSAP-I) exigeant une vibration minimale. Il s'agit de trouver un équilibre optimal entre la rhéologie et le coût initial du nouveau béton pour conférer une bonne performance structurale et économique aux structures. Le programme expérimental établi a premièrement permis d'évaluer la faisabilité d'utilisation des BSAP-I pour la mise en place des piliers d'une infrastructure de pont à Sherbrooke.     Les evaporateurs de refrigeration sont surtout du type tube a ailettes, appeles serpentins, et fonctionnent dans l'une des conditions suivantes: seche, humide ou avec formation de givre. Il a ete demontre que la formation du givre sur la paroi exterieure de l'echangeur engendre une surconsommation energetique a cause des operations de degivrage puisque 15 a 20% seulement de la chaleur produite sert au degivrage tandis que le reste est dissipee dans l'environnement [1]. Avec l'avenement des nouveaux refrigerants, moins nocifs envers l'environnement, l'industrie du froid se trouve penalisee du fait que peu ou pas de composantes mecaniques (compresseur, pompe, echangeur...etc.) adaptees sont disponibles [3]. Il s'agit pour la communaute des frigoristes de combler ce retard technologique en redeveloppant ces composantes mecaniques afin qu'elles soient adaptees aux nouveaux refrigerants. Dans cette optique, et afin de mieux comprendre le comportement thermique des evaporateurs au CO2 fonctionnant dans des conditions seches, qu'un groupe de chercheurs du CanmetENERGIE avaient lance, en 2000, un programme de R & D. Dans le cadre de programme un outil de simulation des evaporateurs au CO2 a ete developpe et un banc d'essai contenant une boucle secondaire de refrigeration utilisant le CO2 comme refrigerant a ete construit. Comme continuite de ce travail de recherche, en 2006 ce meme groupe de recherche a lance un nouveau projet qui consiste a faire une etude theorique et experimentale des evaporateurs au CO2 operants dans des conditions de givrage. Et, c'est exactement dans le cadre de ce projet que se positionne ce travail de these.     Les interactions des electrons de basse energie (EBE) representent un element important en sciences des radiations, particulierement, les sequences se produisant immediatement apres l'interaction de la radiation ionisante avec le milieu biologique. Il est bien connu que lorsque ces radiations deposent leur energie dans la cellule, elles produisent un grand nombre d'electrons secondaires (4 x 104/MeV), qui sont crees le long de la trace avec des energies cinetiques initiales bien inferieures a 20 eV. Cependant, il n'y a jamais eu de mesures directes demontrant l'interaction de ces electrons de tres basse energie avec l'ADN, du principalement aux difficultes experimentales imposees par la complexite du milieu biologique. Dans notre laboratoire, les dernieres annees ont ete consacrees a l'etude des phenomenes fondamentaux induits par impact des EBE sur differentes molecules simples (e.g., N2, CO, O2, H2O, NO, C2H 4, C6H6, C2H12) et quelques molecules complexes dans leur phase solide. D'autres travaux effectues recemment sur des bases de l'ADN et des oligonucleotides ont montre que les EBE produisent des bris moleculaires sur les biomolecules. Ces travaux nous ont permis d'elaborer des techniques pour mettre en evidence et comprendre les interactions fondamentales des EBE avec des molecules d'interet biologique, afin d'atteindre notre objectif majeur d'etudier l'effet direct de ces particules sur la molecule d'ADN. Les techniques de sciences des surfaces developpees et utilisees dans les etudes precitees peuvent etre etendues et combinees avec des methodes classiques de biologie pour etudier les dommages de l'ADN induits par l'impact des EBE. Nos experiences ont montre l'efficacite des electrons de 3--20 eV a induire des coupures simple et double brins dans l'ADN. D'importants travaux ont été consacré à l'utilisation des “matériaux intelligents” comme capteurs et actionneurs. Cette article propose l'analyse de la fiabilité des systèmes mécaniques en étudiant les pannes des actionneurs ou des capteurs. L'effet de ces pannes sur la stabilité et la performance du système y est démontré. Les méthodologies de conception y sont rappelées. Des exemples numériques sont fournis à travers le contrôle d'un panneau sous chargement dynamique pour illustrer la méthode proposée.

A brief description of CEWQO The Central European Workshop on Quantum Optics is a series of conferences started informally in Budapest in 1992. Sometimes small events transform into important conferences, as in the case of CEWQO. Professor Jozsef Janszky, from the Research Institute of Solid State Physics and Optics, is the founder of this series. Margarita Man'ko obtained the following information from Jozsef Janszky during her visit to Budapest, within the framework of cooperation between the Russian and Hungarian Academies of Sciences in 2005. He organized a small workshop on quantum optics in Budapest in 1992 with John Klauder as a main speaker. Then, bearing in mind that a year before Janszky himself was invited by Vladimir Buzek to give a seminar on the same topic in Bratislava, he decided to assign the name 'Central European Workshop on Quantum Optics', considering the seminar in Bratislava to be the first workshop and the one in Budapest the second. This paper describes the preparation of an environmental impact statement, which was initiated as a prelude to the eventual construction and operation of the commercial launching facility on the Island of Hawaii and which follows the Hawaii State law and the National Environmental Policy Act.     Summary Ce travail rétrospectif analyse les particularités épidémiologiques de 543 cas de brûlures d'enfants, représentant 45,7% des admissions de notre centre, en vue de déterminer les éléments pouvant contribuer à renforcer la prévention, qui reste le traitement de choix de cette pathologie. La moyenne d'âge est de 4,25 ans avec une prédilection pour la tranche d'âge d'un à cinq ans, avec 42,5% des cas. Une atteinte masculine est retrouvée dans 63,5% des cas. La brûlure survient à domicile dans 85,1% et accidentellement dans 95% des cas. Les brûlures thermiques représentent 96,5% des causes dominées par les liquides dans 69,3% des cas. La surface cutanée brûlée est ≥ 20% dans 52,3%. La brûlure intéresse essentiellement les membres supérieurs (79,1%). 56,8% des enfants sont transférés par d'autres hôpitaux et le délai de prise en charge hospitalière est supérieur à 6 heures dans 65,5%. In this paper I examine ethical issues relating to the efficacy and safety of these drugs. Pharmaceutical companies have a moral obligation to disclose all information in their possession bearing on the true risks and benefits of their drugs.   8. Utility terrestrial biodiversity issues

    Results from a survey of power utility biologists indicate that terrestrial biodiversity is considered a major issued by only a few utilities; however, a majority believe it may be a future issue. Over half of the respondents indicated that their company is involved in some management for biodiversity, and nearly all feel that it should be a goal for resource management. Only a few utilities are funding biodiversity research, but a majority felt more research was needed. Generally, larger utilities with extensive land holdings had greater opportunities and resources for biodiversity management. Biodiversity will most likely be a concern with transmission rights-of-way construction and maintenance, endangered species issues and general land resource management, including mining reclamation and hydro relicensing commitments. Over half of the companies surveyed have established voluntary partnerships with management groups, and biodiversity is a goal in nearly all the joint projects. Endangered species management and protection, prevention of forest fragmentation, wetland protection, and habitat creation and protection are the most common partnerships involving utility companies. Common management practices and unique approaches are presented, along with details of the survey. 4 refs.

    Cette these est consacree a l'etude de l'evolution structurale des proprietes magnetiques et de transport des multicouches Ni/Fe et nanostructures a base de Co et de l'Ag. Dans une premiere partie, essentiellement bibliographique, nous introduisons quelques concepts de base relies aux proprietes magnetiques et de transport des multicouches metalliques. Ensuite, nous presentons une breve description des methodes d'analyse des resultats. La deuxieme partie est consacree a l'etude des proprietes magnetiques et de transport des multicouches ferromagnetiques/ferromagnetiques Ni/Fe. Nous montrerons qu'une interpretation coherente de ces proprietes necessite la prise en consideration des effets des interfaces. Nous nous attacherons a mettre en evidence, a evaluer et a etudier les effets de ces interfaces ainsi que leur evolution, et ce, suite a des traitements thermiques tel que le depot a temperature elevee et l'irradiation ionique. Les analyses correlees de la structure et de la magnetoresistance nous permettront d'emettre des conclusions sur l'influence des couches tampons entre l'interface et le substrat ainsi qu'entre les couches elles-memes sur le comportement magnetique des couches F/F. La troisieme partie est consacree aux systemes a Magneto-Resistance Geante (MRG) a base de Co et Ag. Nous allons etudier l'evolution de la microstructure suite a l'irradiation avec des ions Si+ ayant une energie de 1 MeV, ainsi que les effets de ces changements sur le comportement magnetique. Cette partie debutera par l'analyse des proprietes d'une multicouche hybride, intermediaire entre les multicouches et les materiaux granulaires. Nous analyserons a l'aide des mesures de diffraction, de relaxation superparamagnetique et de magnetoresistance, les evolutions structurales produites par l'irradiation ionique. Bien que plusieurs recherches rapportent que l'etre humain est de plus en plus conscient de ses impacts sur le climat de la planete, ces memes recherches indiquent qu'en general, les gens continuent a se comporter de facon non ecologique. Que ce soit a l'ecole ou dans la communaute, plusieurs chercheurs en education relative a l'environnement estiment qu'une personne bien intentionnee est capable d'adopter des comportements plus respectueux de l'environnement. Le but de cette these etait de comprendre le processus d'integration de comportements d'attenuation des changements climatiques dans des familles. A cette fin, nous nous sommes fixe deux objectifs : 1) decrire les competences et les procedes qui favorisent l'adoption de comportements d'attenuation des changements climatiques dans des familles et 2) decrire les facteurs et les dynamiques familiales qui facilitent et limitent l'adoption de comportements d'attenuation des changements climatiques dans des familles. Des familles ont ete invitees a essayer des comportements personnels et collectifs d'attenuation des changements climatiques de sorte a integrer des modes de vie plus ecologiques.     This report presents the results of the Community Environmental Response Facilitation Act (CERFA) investigation conducted by The Earth Technology Corporation (TETC) at the Fort Des Moines, a U.S. Government property selected for closure by the Base Realignment and Closure (BRAC) Commission. Under CERFA Federal agencies are required to identify real property that can be immediately reused and redeveloped. Satisfying this objective requires the identification of real property where no hazardous substances or petroleum products, regulated by the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA), were stored for one year or more, known to have been released, or disposed. Fort Des Moines is a 53.28-acre site located in Polk County, Iowa, within the city limits of Des Moines. The installation's primary mission is to provide support and shelter for the U.S. Army Reserve. Activities associated with the property that have environmental significance are photographic processing, vehicle maintenance, printing, and fuel storage. TETC reviewed existing investigation documents; U.S. Environmental Protection Agency (USEPA), State, and county regulatory records; environmental data bases; and title documents pertaining to Fort Des Moines during this investigation. In addition, TETC conducted interviews and visual inspections of Fort Des Moines as well as visual inspections and data base searches for the surrounding properties. Information in this CERFA Report was current as of April 1994.

  13. Etude des phenomenes de penetration des especes chimiques dans les revetements cathodiques des cuves d'electrolyse de l'aluminium

    Cette these decrit l'etude effectuee sur les phenomenes d'insertion du sodium et de penetration du bain electrolytique dans les revetements cathodiques des cuves d'electrolyse de l'aluminium. L'etude fut effectuee a l'aide d'un montage permettant d'effectuer l'electrolyse en laboratoire. Trois types de revetement cathodiques furent etudies : des blocs semi-graphitique, des blocs graphitiques et des blocs graphitises. L'insertion du sodium dans les trois differents types de blocs fut etudiee a l'aide de la spectrometrie des photoelectrons X (XPS). L'analyse a permis de mettre en evidence deux formes de sodium dans l'anthracite et la phase liante des materiaux semi-graphitiques et graphitiques, indiquant qu'une fraction du sodium se retrouve sous forme adsorbee dans les micropores des materiaux alors que l'autre fraction est inseree dans la structure cristalline du carbone. Dans les phases graphitiques (materiaux graphitises et graphitiques), seuls les micropores sont occupes par le sodium Ce resultat explique la tendance observee selon laquelle l'ajout de graphite dans les blocs permet d'abaisser le gonflement sodique. Les mecanismes de penetration du bain dans le reseau poreux des materiaux furent etudies en microscopie electronique et en diffraction des rayons X sur des echantillons apres differents temps d'electrolyses et en variant l'atmosphere (soit sous argon ou sous azote). Ces analyses ont permis d'identifier le mecanisme conduisant a la mouillabilite du bain sur le carbone en fonction de l'atmosphere entourant l'electrolyse. Ainsi, sous azote, la formation de NaCN dans les pores des materiaux par reactions entre le sodium et l'azote permet une mouillabilite accrue du bain alors qu'en absence d'azote (sous argon), le carbure d'aluminium, formes a la surface des pores, joue un role similaire. Dans ce dernier cas, la penetration du bain est moins rapide etant donnee la necessite de toujours amener des especes contenant de l'aluminium en tete du front de

    Coda This issue of the Journal, the last to be produced by the editorial staff in Austin, is remarkable only in the sense that each issue of this Journal is remarkable. In it teachers of chemistry share information, advice, and ideas that encompass a range as wide as chemistry itself. The Journal has always been a venue where the only important thread of commonalty was the goal of helping teachers--providing a "living textbook of chemistry", as it was dubbed by its founders. This textbook is different, however--it is written by its own readers and it has a new "edition" each month. This month's edition typifies the range of topics and issues found important by our readers (and authors): information about new ideas and research advances, new ways to look at and present the information we have, experiments and demonstrations to give students a first-hand exposure to chemistry, computer exercises to introduce those things that cannot be (or are too dangerous to be) presented first-hand, and ideas for better teaching techniques and classroom organization. With such a purview, it is no wonder that the articles in this issue range from the second law of thermodynamics (Frohlich, page 716) to building a Tesla coil from a car coil (Hall and Battino, page 817) and touch on such consequential topics as mimicking nature (Dennison and Harrowven, page 697) and the search for a silicon-based LED (Swisher, Richmond, and Sercel, page 738). New information about research is represented by part III of the series on electrospray ionization mass spectrometry (Hop and Bakhtiar, page A162). Senozan and Devore (page 767) look at data already long known about carbon monoxide poisoning and present a more subtle and complex explanation of the process than is usually seen in introductory texts.     Legal issues worldwide prompted by the AIDS epidemic are discussed, in a general way, since legal systems vary widely in different countries and localities. WHO publishes a tabulation of legal instruments dealing with AIDS and HIV infection. Criminal laws intended to protect people from harm from HIV infection have been enacted, such as a penalty for unprotected sexual intercourse by infected persons, in some Australian states. Knowing spread of HIV already amounts to a crime in many systems. The U.S. Supreme Court has already ruled that states do not violate the constitution for punishing homosexuals for consensual sodomy, nor the Army for discharging homosexuals. Quarantine law is a civil matter, but may provide penalties stricter than criminal penalties, without as much protection. No quarantines against AIDS have been enacted, although some countries require screening of immigrants. Legal issues regarding screening, liability of suppliers of blood products, and tracing of sexual partners are much discussed. Stigmatization of minority and alienated groups such as homosexuals, prostitutes, migrants, drug users and prisoners is a tricky legal problem. The apparent failure of the criminalization of drug users and how to contain the spread of AIDS into the drug free population may prompt drastic new solutions. Other legal issues drawing attention include regulation of health insurance, changes in family law, pre-marriage HIV tests, screening for HIV ostensibly to detect HIV-associated dementia, liability protection for developers and testers of vaccines, and euthanasia and the treatment of the deceased. The legal system tends to lag behind medicine. In the case of AIDS, it cannot afford to delay, therefore effective legal strategies will include effective media presentation of AIDS information to the general public; ready and cheap supply of condoms; and a new approach to illegal drugs.     Les transplantations multi-organes, comprenant les blocs foie-duodénum-pancréas, foie-estomac-duodénum-pancréas, et foie-intestin sont réalisées avec un succés croissant Ces techniques et leurs combinaisons variées de transplantation monobloc ne sont pas de pratique courante. Les techniques de prélévement, de conservation et de soins post-opératoires sont décrites pour la transplantation multi-organes compléte ainsi que pour les variantes incomplétes. Le probléme particulier à ce type de transplantation est celui de la transplantation intestinale, c’est-à-dire la transplantation d’un organe à composante lymphoréticulaire complexe ce qui peut provoquer un syndrome greffon contre hôte. Par erreur de conception, et un peu par esprit de systéme, les efforts par le passé étaient dirigés sur la modification et la destruction des systémes lymphoréticulaires grâce au traitement préalable du donneur ou des organes transplantés, par médicaments, radiation ou autres moyens. Actuellement, I’idée directrice est de garder intacte les systémes lymphoréticulaires qui deviennent alors le site d’une circulation à double sens aprés transplantation. Avec la puissante immunodépression que fournit le FK 506, les cellules lymphoréticulaires du donneur peuvent circuler chez le receveur sans créer de syndrome du greffon contre hôte clinique et les cellules de la greffe s’assimilent à celles du receveur (chimérisme local) sans provoquer de rejet. Même si I’on évite le rejet ou le syndrome greffon contre hôte, il existe, à côté de ces entités, des relations métaboliques entre les organes greffés ainsi qu’entre les organes greffés et les viscéres du receveur laissés en place, qui peuvent influencer I’avenir soit des organes greffés, soit des organes laissés en place.   19. Authoring Issues beyond Tools

    Authoring is still considered a bottleneck in successful Interactive Storytelling and Drama. The claim for intuitive authoring tools is high, especially for tools that allow storytellers and artists to define dynamic content that can be run with an AI-based story engine. We explored two concrete authoring processes in depth, using various Interactive Storytelling prototypes, and have provided feedback from the practical steps. The result is a presentation of general issues in authoring Interactive Storytelling, rather than of particular problems with a specific system that could be overcome by 'simply' designing the right interface. Priorities for future developments are also outlined.

The persistence of this disease is caused by a large pool of immune-compromised and lower socioeconomic populations. The advent of rapid transportation and migration has contributed to the persistence of this disease in developed and less developed nations. The emergence of drug-resistant strains has added an additional factor for the pervasiveness of tuberculosis. The genitourinary system is a primary target for hematogenous infections. Brady, Commerce Assistant Secretary for Trade Administration, at Congressional hearings on the national security issues of technology transfers to the Soviet Union identified steps the US needs to take to deal effectively with the problem. These steps include an understanding of how the Soviet Union has and will benefit militarily by acquiring Western technology and efforts to work with other countries, counterintelligence agencies, and industries to stem the flow of technological information. Brady outlined changes in technology development that complicate the enforcement of transfer rules, and emphasized the importance of a close relationship between the business community and the Commerce Department.     the base and derived units of physics still being open to debate. Giving weight and coherence to this issue is the collective experience of the authors. Individually, they have made important contributions to measurement, have taken part in the operations of national standards laboratories and in the work in which such laboratories collaborate. They are members of the committees which negotiate terms for the use and recognition, at national, regional and international level, of the units which industrial laboratories maintain. Just under half of them are members of the Comité Consultatif des Unites (CCU), the committee to which the Comité International des Poids et Mesures (CIPM), itself turns when it looks for advice on issues affecting the use of units. This issue deals with topics which are central to the decisions of the CIPM. This is an appropriate place, therefore, to mention the close interest which two members of the Comité International have taken in Metrologia and the unfailing support they have given it: Professor Dieter Kind and Professor Jan de Boer. As President of the Comité International, Professor Kind greatly influenced the decision that the Bureau International des Poids et Mesures should purchase Metrologia and did much to create the spirit of independent commentary in which the journal operates.     We establish the connection between the Magellanic Clouds (MCs) and the dwarf galaxy candidates discovered in the Dark Energy Survey (DES) by building a dynamical model of the MC satellite populations, based on an extensive suite of tailor-made numerical simulations. Our model takes into account the response of the Galaxy to the MCs infall, the dynamical friction experienced by the MCs and the disruption of the MC satellites by their hosts. The simulation suite samples over the uncertainties in the MC's proper motions, the masses of the MW and the Clouds themselves, and allows for flexibility in the intrinsic volume density distribution of the MC satellites. As a result, we can accurately reproduce the DES satellites' observed positions and kinematics. Assuming that Milky Way (MW) dwarfs follow the distribution of sub-haloes in Λ cold dark matter, we further demonstrate that, of 14 observed satellites, the MW halo contributes fewer than 4(8) of these with 68(95) per cent confidence and that 7(12) DES dwarfs have probabilities greater than 0.7(0.5) of belonging to the Large Magellanic Cloud (LMC). Marginalizing over the entire suite, we constrain the number of Magellanic satellites in the range -7 < MV < -1 which exceed the DES surface brightness threshold at ˜70, and the mass of the LMC around 1011 M⊙. The data also strongly support a first-infall scenario for the LMC. Finally, we give predictions for the line-of-sight velocities and the proper motions of the satellites discovered in the vicinity of the LMC.

    The Dark Energy Survey Supernova program (DES SN) has already discovered over 1000 Type Ia supernovae with well-sampled multi-color light curves in the redshift range 0.1 < z < 1.2. I will present an overview of the survey and show recent advances in our detection, photometry, calibration and spectroscopic follow-up pipelines. I will go over initial results from photometric classification of our sample and discuss methods used to reach measurements of cosmological parameters.

Jahrhunderts bis heute stetig weiterentwickelt [2]. In neuerer Zeit steigt die Anzahl komplexer Anwendungen, die die gezielte Kombination verschiedener Funktionalitäten in einem Formteil erfordern. Das Standard-Spritzgießen kann diese Anforderungen immer weniger befriedigen. Daher gewinnen die Sonderverfahren des Spritzgießens zunehmend an Bedeutung [3]. Ihre Anzahl beträgt inzwischen über 100. Die Aufgabe des Anwenders ist es, aus der Vielzahl der möglichen Verfahren, ein anforderungsgerechtes auszuwählen, das sowohl unter technischen wie wirtschaftlichen Gesichtspunkten die optimale Lösung darstellt. Dies setzt die ständige Auseinandersetzung mit Entwicklungstendenzen im Bereich der Spritzgießtechnologie voraus.     Les enfants tananariviens sont en état de malnutrition chronique. Notre objectif est d’évaluer l'indice de masse corporelle (IMC) pour estimer les enfants apparemment "sains". Une enquête et une mesure de la taille et du poids des enfants scolarisés tananariviens de 6 à 11 ans ont été réalisées. Après leur accord, la taille et l'indice de masse corporelle des 442 enfants tirés au hasard ont été ainsi obtenus. L'analyse de la moyenne de la taille a révélé une différence significative à 8 ans, une différence non évidente sur l'indice de masse corporelle. La comparaison avec les valeurs de référence (OMS 2006) a montré un retard statural de 34% avec une tendance globale à la hausse et un déficit pondéral égal à 5,5% selon le z score. Ainsi, au sein d'une population malnutrie, l'indice de masse corporelle pourrait être utilisé comme un des paramètres à considérer dans l’évaluation de l’état de santé pour classer ces enfants en bonne santé apparente.     La panniculite est une maladie inflammatoire du tissu adipeux sous-cutané rarement associée à la dermatomyosite. Elle peut survenir avant, après ou en même temps que l'atteinte musculaire. Dans la plupart des cas, l’évolution de la panniculite et des autres atteintes de la dermatomyosite est favorable sous traitement corticoïde et/ou immunosuppresseur. Nous rapportons le cas d'une patiente âgée de 48 ans ayant présenté des lésions de panniculite précédant de 2 mois les signes musculaires. L'atteinte cutanée était résistante au traitement corticoïde associés aux immunosuppresseurs ce qui a nécessité le recours au traitement par Immunoglobulines polyvalentes permettant ainsi une amélioration à la fois de l'atteinte cutanée et musculaire.

    Global concern about the environment is increasing, and the People's Republic of China (PRC) is not immune from such concerns. The Chinese face issues similar to those of many other developing nations. The US Department of Energy is particularly interested in national and world pollution issues, especially those that may infringe on other countries' economic growth and development. The DOE is also interested in any opportunities that might exist for US technical assistance and equipment in combating environmental problems. Our studies of articles in the China Daily, and English-language daily newspaper published by the Chinese government, show that population, pollution, and energy are major concerns of the Chinese Communist Party. Thus this report emphasizes the official Chinese government view. Supporting data were also obtained from other sources. Regardless of the severity of their various environmental problems, the Chinese will only try to remedy those problems with the greatest negative effects on its developing economy. They will be looking for foreign assistance, financial and informational, to help implement solutions. With the Chinese government seeking assistance, the United States has an opportunity to export basic technical information, especially in the areas of pollution control and monitoring, oil exploration methods, oil drilling technology, water and sewage treatment procedures, hazardous waste and nuclear waste handling techniques, and nuclear power plant safety procedures. In those areas the US has expertise and extensive technical experience, and by exporting the technologies the US would benefit both economically and politically. 59 refs., 3 figs.

    One year ago, a panel of industry experts were interviewed in the November/December 1994 issue of Fuel Reformulation (Vol. 4, No. 6). With the focus then and now on refinery investments, the panelists were asked to forecast which refining processes would grow in importance. It is apparent from their response, and from other articles and discussions throughout the year, that hydroprocessing and catalytic conversion processes are synergistic in the overall refinery design, with flexibility and process objectives varying on a unit-by-unit case. To an extent, future refinery investments in downstream petrochemicals, such as for paraxylene production, are based on available catalytic reforming feedstock. Just a importantly, hydroprocessing units (hydrotreating, hydrocracking) needed for clean fuel production (gasoline, diesel, aviation fuel), are heavily dependent on hydrogen production from the catalytic reformer. Catalytic reforming`s significant influence in the refinery hydrogen balance, as well as its status as a significant naphtha conversion route to higher-quality fuels, make this unit a high-priority issue for engineers and planners striving for flexibility.

    RÉSUMÉ Contexte : La présence d’auteurs honorifiques et fantômes ainsi que les intérêts concurrents représentent des difficultés bien documentées, liées à la publication d’articles scientifiques. Il existe des lignes directrices encadrant la rédaction et la publication de manuscrits scientifiques, notamment celles de l’International Committee of Medical Journal Editors (ICMJE). Objectifs : L’objectif principal de cette étude descriptive et transversale visait à recenser les instructions portant sur la paternité des articles et les intérêts concurrents provenant des recommandations aux auteurs des journaux traitant de pratique pharmaceutique. L’objectif secondaire visait à déterminer des mesures correctrices pour une paternité des articles plus transparente. Méthode : La recherche a débuté par l’identification des journaux traitant de pratique pharmaceutique. La consultation des instructions aux auteurs des journaux a permis ensuite de recenser les recommandations destinées à éviter les problèmes de paternité des articles et d’intérêts concurrents. Finalement, les membres de l’équipe de recherche se sont consultés afin de définir des mesures correctrices possibles à l’intention des chercheurs. Résultats : Des 232 journaux traitant de pharmacie, 33 ont été définis comme traitant de pratique pharmaceutique. Un total de 24 (73 %) journaux mentionnaient suivre la politique de l’ICMJE, 14 (42 %) demandaient aux auteurs de remplir un formulaire de déclaration d’intérêts concurrents au moment de la soumission de l’article, 17 (52 %) présentaient une définition de la qualité d’auteur et 5 (15 %) demandaient de détailler les contributions de chaque auteur. Une grille de 40 critères a été élaborée pour définir l’attribution du statut d’auteur.     There are various types of hydraulic turbines. Regarding the operating conditions, geometries and technologies differ. Hydraulic seals are only used in Francis turbines, which are widely used. The role of hydraulic seals is not to be waterproof. Their main aim is to prevent contact between the rotating and static parts of the turbine. Although necessary, hydraulic seals create energetic losses : some fluid does not flow through the runner (leakage loss) and exerts a torque on the rotor (friction loss). In a context of constant progression towards still more efficient turbines, the optimization of each part of the turbine is necessary. Our study is a part of this research seeking to decrease the losses in turbines as much as possible. In order to understand fully the problem, and to ensure an optimal seal exists, an analytical study has been lead in the first place. It establishes the analytical expressions of the speeds, pressure, losses and optimal seal length for laminar flows in straight seals. Various tests were then lead with the ANSYS CFX solver in order to highlight aspects which necessitate a particular attention. For example, the issues of boundary conditions and dimensionless simulations were adressed. A CFD model has then been validated. The results of the experiences lead in the sixties by Dominion Engineering Works, which later became Andritz Hydro Limited, were used in this process. Even if all the tests were not useable, some of them were reproduced numerically. The CFD model which was used features SST turbulence modeling, 2D axisymmetric geometries, parabolic mesh distributions, smooth walls, and a outlet headloss based on the normal speed. For the various tests which were considered, the average discrepancy between numerical and experimental results is 6.5%.   17. Traitement des fractures des plateaux externes par vissage percutané assisté par arthroscopie

    Le but de notre étude est d'évaluer les résultats de fractures des plateaux tibiaux externes traitées par ostéosynthèse percutanée assistée par arthroscopie. Dix patients (8 hommes et 2 femmes) de 32 ans en moyenne ont subi cette intervention afin de réparer des fractures des plateaux tibiaux Schatzker I-III. Après avoir appliqué un garrot pneumatique, nous avons réduit et fixé la fracture au moyen de vis cannelées souschondrales. Lésions associées retrouvent deux lésions partielles du ménisque externe ont été retrouvé, traitées par résection partielle. Une orthèse de genou été de mise à but antalgique et protectrice pendant six semaines avec béquillage et interdiction de l'appui pour une durée de douze semaines avec reprise d'appui partiel au delà. La durée d'hospitalisation été d'une moyenne de cinq jours. La rééducation passive a été commence le lendemain de l'intervention et continuait dans chez un kinésithérapeute à la sortie du patient du service. Le suivi été à J7, J15, 1mois, 3mois, 6 mois puis tous les 6 mois. Neuf de nos patients ont été revu régulièrement sauf un perdu de vue. Le recul moyen de notre série été de 16 mois (10 et 24 mois). Le score de Lysholm a été utilisé pour évaluer les résultats cliniques chez nos neuf patients: excellent chez trois patients bons chez trois moyen chez un seul et mauvais chez deux patients. Tous nos neuf patients ont consolidé (figure 10 contrôle scopique d un article). Aucune gonarthrose n'a été note chez nos neuf patients due essentiellement au recul moyen faible de 16 mois. Le traitement des fractures des plateaux tibiaux externes assisté par arthroscopie produit des résultats satisfaisants et peut être accepté comme solution de rechange efficace au traitement des fractures des plateaux tibiaux causées par un choc de faible énergie.   18. Broadband network selection issues

    Selecting the best network for a given cable or telephone company provider is not as obvious as it appears. The cost and performance trades between Hybrid Fiber Coax (HFC), Fiber to the Curb (FTTC) and Asymmetric Digital Subscriber Line networks lead to very different choices based on the existing plant and the expected interactive subscriber usage model. This paper presents some of the issues and trades that drive network selection. The majority of the Interactive Television trials currently underway or planned are based on HFC networks. As a throw away market trial or a short term strategic incursion into a cable market, HFC may make sense. In the long run, if interactive services see high demand, HFC costs per node and an ever shrinking neighborhood node size to service large numbers of subscribers make FTTC appear attractive. For example, thirty-three 64-QAM modulators are required to fill the 550 MHz to 750 MHz spectrum with compressed video streams in 6 MHz channels. This large amount of hardware at each node drives not only initial build-out costs, but operations and maintenance costs as well. FTTC, with its potential for digitally switching large amounts of bandwidth to an given home, offers the potential to grow with the interactive subscriber base with less downstream cost. Integrated telephony on these networks is an issue that appears to be an afterthought for most of the networks being selected at the present time. The major players seem to be videocentric and include telephony as a simple add-on later. This may be a reasonable view point for the telephone companies that plan to leave their existing phone networks untouched. However, a phone company planning a network upgrade or a cable company jumping into the telephony business needs to carefully weigh the cost and performance issues of the various network choices.     The number of people with HIV/AIDS continues to increase globally. Women, who represent the subgroup with the fastest rate of increase, are usually informed of their serostatus by the obstetrician/gynaecologist. As treatment of infected women raises a number of ethical issues, an understanding of the theoretical background for ethical decision making is requisite to ensure these problems are resolved within a morally appropriate framework. Vigorous debate has arisen from the tensions between the competing goals of HIV testing, third party disclosure, management of the critically ill HIV-infected woman, infertility management in the background of HIV/AIDS, and gender-based violence as cause or result of acquiring HIV infection. Women may be differently empowered economically, socially and culturally. What may be a satisfactory solution in the context of the USA and Europe may be far from ideal in that of the developing world. PMID:15778114

We move from this inquiry into the nature of emotion to an inquiry into their epistemology. Do they provide reasons for evaluative judgments and, more generally, do they contribute to our knowledge of values? We then address the question of the social dimension of emotions, explaining how the traditional nature versus nurture contrast applies to them. We finish by exploring the relations between emotions, motivation and action, concluding this overview with a more specific focus on how these relations bear on some central ethical issues.     The epidemic of substance abuse continues to pose a significant challenge to clinicians nationwide. Although there is a tendency to simply associate drug abuse with poverty, the problem affects every social stratum gender and race; and pregnant women are no exception. Caring for pregnant, substance-using women and their infants presents complex legal and ethical issues. Debate is ongoing about whether criminal penalties should be imposed on women based solely on their use of alcohol and other drugs during pregnancy. Furthermore, controversies persist about the rights and wishes of pregnant women versus the interests of their fetuses. For health professionals, conflict arises when the pregnant woman chooses behaviors that have the potential to harm the developing fetus. The ethical dilemma arises from competing autonomy-based and beneficence-based obligations to the maternal-fetal dyad. This chapter explores the ethics-based conflicts in the delivery of health care to drug abusing pregnant women. PMID:20407974

    This paper reviews the general background of health risks associated with mercury (Hg), primarily methylmercury (MeHg), with a view towards application to advanced technologies that could reduce any contributions from coal combustion. The need for accurate assessment of such risks is discussed, since Hg is now widely dispersed in the environment and cannot easily be eliminated. The primary pathway of MeHg intake is through eating contaminated fish. The issues of concern include identification of critical health outcomes (various neurological indices) and their confounding factors, accurate assessment of MeHg intake rates, and appropriate use of dose-response functions. Ultimately, such information will be used to evaluate alternative coal combustion systems.

    Migration and urbanization are issues that require increasing attention in Mongolia. Mr. Sodov Sonin, Minister of Health and Social Welfare, stated at the Forum that fertility has declined, but mortality, in particular the mortality of children and mothers, is still too high. In addition, there is a significant gap between the knowledge of and behaviors concerning reproductive health, which is one of the causes of the country's high abortion rates. However, on the positive side, literacy is high among women--70% of the students in Mongolia's higher educational institutions are female and the State recognizes equal rights for women. Moreover, programs that promote health and education, including the National Program on Reproductive Health, are being implemented; but despite all these, Mongolia still lacks the human and financial resources to implement the ICPD Program of Action satisfactorily. The country also needs dramatic changes in mind-set and in terms of capacity building, given its ongoing socioeconomic transition. PMID:12295512

    The United Nations Decade for the Advancement of Women, from 1975 to 1985, leaves a legacy of a deeper understanding of the issues, and the emergence of new networks with the experience and commitment to work for further changes. However, the role and status of women did not improve. There is a new commitment to struggle for the ending of all oppression, injustice and violence of all kinds at all levels. Feminism is a consciousness of all forms of women's oppression and a commitment to work against them. Feminist critiques illuminate the larger structures that oppress both women and men. New development theories embracing feminism are necessary to understand how patriarchy and economic systems propogate oppression. The production-oriented approach to rural development is flawed n failing to address women's lack of access to land, credit, training and new technologies. Overwhelming household tasks, cultural norms, and traditional attitudes limit women's involvement in training programs and other development activities. The basic needs approach to rural development provides access to vital services to meet a family's basic needs for nutrition, housing and clothing, and allows people's participation in decision making. However, women have little actual role in decision making so their needs, concerns and perspectives are not taken into account. Women are treated as instruments to achieve goals without appreciating their perspective. Project-based approached emphasize short term goals rather than laying the foundation for longterm changes. Few projects address structural issues or empower women. Projects must include education to increase personal growth and self reliance. Development planning can be enormously enhanced by taking gender differences into account and recognizing that people, specially poor women, can promote their own devleopment.     As opposed to the well developed and understood equity markets, the energy markets are still in their infancy. The explosion of contracts, of both the primary and derivative types, are testament both to the existing size and the untapped growth potential of this exciting industry. However, because of its relative youth many basic issues in the energy markets remain unresolved. This thesis introduces some interesting questions and provides insights into these issues. Thematically, the chapters of this thesis are linked by an emphasis on valuation and risk management decisions. A contribution of this thesis is to show that subtle differences between the endogenous price process in our general equilibrium setting and the exogenous processes considered in earlier papers can generate significant differences in both financial and real option values. In addition to these valuation concerns there has been much debate about the corporate risk management function. Finance theory suggests that a value maximizing corporation should either be indifferent to hedging or, in the presence of certain imperfections, it should completely hedge all exposures. Both these extremes contradict empirical evidence. We show that existing corporate hedging behaviour is best explained in light of both physical market imperfections and directional predictions on future prices. While these speculative motives may arise from corporate hubris or genuine informational advantages, we argue that it would be difficult to implement private information in the absence of noise traders. Related to the risk management decision is the existence of futures risk premia. These premia have been thought to be cause by covariance with priced factors or due to the hedging demands of consumers and producers. This thesis argues that inventories serve as a signal of available quantity, which coupled with consumers fears regarding stockouts can induce a positive relationship between premia and inventories.     Les premieres etudes d'antennes a base de supraconducteurs a haute temperature critique emettant une impulsion electromagnetique dont le contenu en frequence se situe dans le domaine terahertz remontent a 1996. Une antenne supraconductrice est formee d'un micro-pont d'une couche mince supraconductrice sur lequel un courant continu est applique. Un faisceau laser dans le visible est focalise sur le micro-pont et place le supraconducteur dans un etat hors-equilibre ou des paires sont brisees. Grace a la relaxation des quasiparticules en surplus et eventuellement de la reformation des paires supraconductrices, nous pouvons etudier la nature de la supraconductivite. L'analyse de la cinetique temporelle du champ electromagnetique emis par une telle antenne terahertz supraconductrice s'est averee utile pour decrire qualitativement les caracteristiques de celle-ci en fonction des parametres d'operation tels que le courant applique, la temperature et la puissance d'excitation. La comprehension de l'etat hors-equilibre est la cle pour comprendre le fonctionnement des antennes terahertz supraconductrices a haute temperature critique. Dans le but de comprendre ultimement cet etat hors-equilibre, nous avions besoin d'une methode et d'un modele pour extraire de facon plus systematique les proprietes intrinseques du materiau qui compose l'antenne terahertz a partir des caracteristiques d'emission de celle-ci. Nous avons developpe une procedure pour calibrer le spectrometre dans le domaine temporel en utilisant des antennes terahertz de GaAs bombarde aux protons H+ comme emetteur et detecteur. Une fois le montage calibre, nous y avons insere une antenne emettrice dipolaire de YBa 2Cu3O7-delta . Un modele avec des fonctions exponentielles de montee et de descente du signal est utilise pour lisser le spectre du champ electromagnetique de l'antenne de YBa 2Cu3O7-delta, ce qui nous permet d'extraire les proprietes intrinseques de ce dernier.     The development of renewable energies is growing over the last decade to face environmental issues due to the world fossil fuel consumption increase. These energies are highly involved in houses and commercial buildings and numerous systems have been proposed to meet their energy demand. Therefore, improving both efficiency and use of systems, i.e. improving energy management, appears essential to limit the ecological footprint of humanity on the planet. However, system integration yields a very complex problem to be solved due to the large number of units and theirs technology, size, working conditions and interconnections. This situation highlights the lack of systematic analysis for comparing integrated system performance and for correctly pointing out their potential. As a result, the objective of this thesis is to develop and to present such a method, in other words the structural optimization of energy systems. It will be helpful to choose the optimal equipment by identifying all the possibilities of system arrangements and for comparing their performance. Combinations have then been subjected to environmental (climate), structural (available area) and economical constrains while assessment criteria have considered both energy, economic and ecological aspects. For that reason, as well as energy and economic analyses, the exergy concept has also been applied to the equipment. Nevertheless, the high degree of complexity of integrated systems and the tedious numerical calculations make the resolution by using standard software very difficult. It is clear that the whole optimization project would be considerable and the aim is to develop models and optimization tools. First of all, an exhaustive review of energy equipment including photovoltaic panels, solar collectors, heat pumps and thermal energy storage systems, has been performed.     Composing the Curriculum For your summer reading pleasure this month's issue provides a wide range of subjects and activities. Our lead article is a tribute to a chemist-composer, Lejaren A. Hiller, Jr., as well as an examination of the interesting relationship between chemistry (and science) and music. Like Hiller, Wamser and Wamser (page 601) love both chemistry and music, and this comes through loud and clear in their historical perspective on Hiller's work. Much of the rest of the issue impinges on the chemistry curriculum in one form or another. Rettich, Bailey, Frank, and Frick (page 638) describe an integrated first- and second-year curriculum being developed at Illinois Wesleyan University that includes all subdisciplines of chemistry. They describe ways to teach topics such as acids and bases so that the perspectives of (for example) organic and inorganic chemistry are integrated and contemporaneous. Those contemplating a popular reform, group work or cooperative learning, will be interested in Birk and Kurtz's program (page 615) for training teaching assistants using cooperative learning. Surely TAs who have participated in cooperative groups to develop their own techniques for handling discussion and lab sessions will be able to use group learning to better advantage. Several years ago the ACS Division of Chemical Education set up a Task Force on the General Chemistry Curriculum. With support from the National Science Foundation and others, the task force has been quite active.     The symptoms associated with exposure to ionizing radiation are nausea, vomiting, diarrhoea. The response of the gut is complex involving modifications of motility and fluid and electrolyte transport. Gastrointestinal regulatory peptides have an important role in these functions. This study showed that radiation-induced tissue variations of neuropeptides have some repercussions on intestinal biological activity of these peptides soon after irradiation. In addition such modifications are also seen a few years after irradiation. Les symptômes associés à l'exposition aux rayonnements ionisants sont des nausées, vomissements et diarrhées. La réponse du système digestif est complexe, impliquant des modifications de la motilité et du transport d'eau et d'électrolytes. les neuropeptides gastro-intestinaux ont un rôle important dans ces fonctions. Cette étude montre que les variations tissulaires de ces neuropeptides induites par l'irradiation ont des répercussions sur l'activité biologique intestinale pour des temps précoces mais que ces perturbations sont encore visibles quelques années après l'irradiation.

    Le graphene est un materiau a base de carbone qui est etudie largement depuis 2004. De tres nombreux articles ont ete publies tant sur les proprietes electroniques, qu'optiques ou mecaniques de ce materiel. Cet ouvrage porte sur l'etude des fluctuations de conductance dans le graphene, et sur la fabrication et la caracterisation de nanostructures gravees dans des feuilles de ce cristal 2D. Des mesures de magnetoresistance a basse temperature ont ete faites pres du point de neutralite de charge (PNC) ainsi qu'a haute densite electronique. On trouve deux origines aux fluctuations de conductance pres du PNC, soit des oscillations mesoscopiques provenant de l'interference quantique, et des fluctuations dites Hall quantique apparaissant a plus haut champ (>0.5T), semblant suivre les facteurs de remplissage associes aux monocouches de graphene. Ces dernieres fluctuations sont attribuees a la charge d'etats localises, et revelent un precurseur a l'effet Hall quantique, qui lui, ne se manifeste pas avant 2T. On arrive a extraire les parametres caracterisant l'echantillon a partir de ces donnees. A la fin de cet ouvrage, on effectue des mesures de transport dans des constrictions et ilots de graphene, ou des boites quantiques sont formees. A partir de ces mesures, on extrait les parametres importants de ces boites quantiques, comme leur taille et leur energie de charge.

    Sulfides present in hornsfeld aggregates, used in light pavanent construction in contact with humid air alter rapidly. Crystallisation of sulfates at the interface of bitumastic material and sub-base creates serious problems (intumescence and crazing) that can lead to a total reconstruction of the project (roads, sidewalks, sports areas, etc). The sulfides in the aggregates and the sulfates produced due to alteration are studied by SEM and XRD. The results show that the intensity of this phenomenon is linked to the nature and the crystallinity of the sulfides. The evolution of the sulfates formed during this alteration process is slow and complex. In new pavements (3 years) the sulfates have a pulverised appearance and consist mostly of epsomite, associated with pickeringite and halotrichite. In older pavements (20 years) the sulfates form a fibrous concretion consisting of pickeringite and small quantities of halotrichite. Les sulfures présents dans les granulats élaborés à partir de cornéen nes s'altèrent rapidement dans les chaussées légères en présence d'air humide. La cristallisation des sulfates à l'interface enrobé-couche de fondation crée des désordres si importants (intumescences, faiençage) qu'il est bien souvent nécessaire d'assurer la réfection totale des ouvrages (routes, trottoirs, plateaux sportifs...).     The role of psychological factors in IVF is complex. Psychological issues intertwine with physical ones, often with additive effects. The very diagnosis of infertility is likely to cause stress. In addition, the many investigations and procedures may have compounded distress. There are probably a small number of patients in whom psychological factors may induce infertility. But in the majority, psychological factors may exacerbate infertility and influence the patient's and partner's responses. Mental, sexual, marital and social adjustment may all be affected. The procedure of IVF is likely to have a further impact. A pilot study of couples entering an IVF programme revealed the women to be highly anxious and to conform strongly to feminine stereotypes. Many had received psychiatric help in the past. The idiopathic group appeared to cope less well with stress and had higher anxiety and neuroticism scores. Follow-up revealed that IVF had a profound impact on many of the women. Most had received no counselling in the interim. In those who completed questionnaires at follow-up, a differential effect was observed between the organic and idiopathic groups. State anxiety fell in the idiopathic group but so did marital adjustment. The clinician is advised to incorporate consideration of the psychological aspects of IVF into every aspect of the programme. The addition of a psycho-social team may assist the gynaecologist in this and help the couple to make an optimal adjustment. PMID:3833441

  7. Nanofluids Research: Key Issues

    Nanofluids are a new class of fluids engineered by dispersing nanometer-size structures (particles, fibers, tubes, droplets) in base fluids. The very essence of nanofluids research and development is to enhance fluid macroscopic and megascale properties such as thermal conductivity through manipulating microscopic physics (structures, properties and activities). Therefore, the success of nanofluid technology depends very much on how well we can address issues like effective means of microscale manipulation, interplays among physics at different scales and optimization of microscale physics for the optimal megascale properties. In this work, we take heat-conduction nanofluids as examples to review methodologies available to effectively tackle these key but difficult problems and identify the future research needs as well. The reviewed techniques include nanofluids synthesis through liquid-phase chemical reactions in continuous-flow microfluidic microreactors, scaling-up by the volume averaging and constructal design with the constructal theory. The identified areas of future research contain microfluidic nanofluids, thermal waves and constructal nanofluids. PMID:20676214

    It is a relevant issue in the research on laser propulsion that experimental results are treated seriously and that meaningful scientific comparison is possible between groups using different equipment and measurement techniques. However, critical aspects of experimental measurements are sparsely addressed in the literature. In addition, few studies so far have the benefit of independent confirmation by other laser propulsion groups. In this paper, we recommend several approaches towards standardization of published laser propulsion experiments. Such standards are particularly important for the measurement of laser ablation pulse energy, laser spot area, imparted impulse or thrust, and mass removal during ablation. Related examples are presented from experiences of an actual scientific cooperation between NU and DLR. On the basis of a given standardization, researchers may better understand and contribute their findings more clearly in the future, and compare those findings confidently with those already published in the laser propulsion literature. Relevant ISO standards are analyzed, and revised formats are recommended for application to laser propulsion studies.

She reflects upon in detail: (1) the logistics of using SPs in teaching; (2) how SPs are used in assessment; (3) what aspects of performance SPs can be trained to record and evaluate; (4) issues concerning checklists; (5) evaluation of interviewing skills; (6) evaluation of written communication skills; (7) importance of defining what is being tested; (8) various kinds and uses of inter-station exercises and problems of scoring them; (9) case development and the various sources for case material; (10) ways to generate scores; (11) selecting and training SPs; (12) role of the faculty and primary importance of bedside training with real patients; and (13) pros and cons of national versus single-school efforts to use SPs.   10. Reagan issues mineral policy

    The National Materials and Minerals Program plan and report that President Reagan sent to Congress on April 5 aims to ‘decrease America's minerals vulnerability’ while reducing future dependence on potentially unstable foreign sources of minerals. These goals would be accomplished by taking inventory of federal lands to determine mineral potential; by meeting the stockpile goals set by the Strategic and Critical Material Stockpiling Act; and by establishing a business and political climate that would encourage private-sector research and development on minerals.Now that the Administration has issued its plan, the Subcommittee on Mines and Mining of the House Committee on Interior and Insular Affairs will consider the National Minerals Security Act (NMSA), which was introduced 1 year ago by subcommittee chairman Jim Santini (D-Nev.) [Eos, May 19, 1981, p. 497]. The bill calls for establishing a three-member White-House-level council to coordinate the development of a national minerals policy; amending tax laws to assist the mining industry to make capital investments to locate and produce strategic materials; and creating a revolving fund for the sale and purchase of strategic minerals. In addition, the NMSA bill would allow the secretary of the interior to make previously withdrawn public lands available for mineral development. The subcommittee will hold a hearing on the Administration's plan on May 11. Interior Secretary James Watt has been invited to testify.

    This paper reports results from guideway analyses conducted as part of the National Maglev Initiative (NMI), a government-industry effort from 1989 to 1994, formed to encourage the development of US maglev technology and to assess its potential application within the US transportation system. Covered here are some key guideway design issues that were common to the designs assessed for the NMI, and to maglev guideways in general. They represent aspects that will need additional attention in future efforts to produce structurally sound and economical maglev guideways. These recommendations come from the analyses conducted by a team from the US Army Construction Engineering Research Laboratories, the Civil Engineering Department of the University of Illinois, and Alfred Benesch and Company. The recommendations focus on design philosophy and the development of general design criteria, guideway maintenance and the provision for future alignment adjustment in both the guideway and the magnets, foundation design, and the long-term performance of guideway materials and reinforcement. Generally, one of the main challenges to guideway designers is to produce a structure that will be easily maintainable to the narrow tolerances and precise alignment required for practical high-speed maglev operation.

    The objective of the Solid Propellant Environmental Issues (SPEI) project is to demonstrate environmentally acceptable technologies that will enhance the continued production of solid rocket motors (SRMs) by complying with current and anticipated environmental regulations. Phase 1 of the project identifies current and anticipated environmental regulations that may affect SRMs manufacturing in the future and identify emerging process technologies which comply with these regulations. Phase 2 of the project established a baseline database by fabricating a 363 kg motor using the current manufacturing process. In Phase 3, environmentally acceptable process technologies were evaluated, ranked, and selected for demonstration using criteria developed by the team. The results for Phase 1--3 have previously been presented. This paper will present data obtained to date on Phase 4. In Phase 4, the alternate process technologies were evaluated for compatibility, cleaning effectiveness, and waste minimization/pollution prevention. The best performing candidate for each application area was selected for demonstration. The selected process technologies will be inserted into the baseline manufacturing process from Phase 2. The new manufacturing process will be demonstrated and evaluated through the scale-up and fabrication of two 363 kg solid rocket motors.

On a pu reproduire par hypertrempe puis laminage entre 400^{circ}C et 600^{circ}C une microstructure similaire. Des essais mécaniques (traction, fluage, relaxation, sur éprouvettes lisses et pré-fissurées) ont été réalisés à 550^{circ}C et à 600^{circ}C sur ces zones affectées simulées et sur un état de référence hypertrempé. Ils ont montré que l'écrouissage diminuait la ductilité dans le domaine de rupture intergranulaire, sans modifier qualitativement le mécanisme d'endommagement. Pendant la pré-déformation les incompatibilités de déformation entre grains conduiraient à l'existence de contraintes locales élevées qui favoriseraient la germination des cavités intergranulaires.

Même si les démarches thérapeutiques actuelles varient au Canada et ailleurs, les données jusqu’à présent indiquent que le traitement familial (TF) est le plus efficace pour les enfants et les adolescents anorexiques. Un élément essentiel du modèle de TF, c’est que les parents sont investis de la responsabilité de rétablir la santé physique de leur enfant et de s’assurer de la reprise complète de son poids. Le médecin qui comprend les principes fondamentaux et la philosophie du TF peut mettre en place les éléments de cette intervention fondée sur des faits probants auprès des jeunes patients anorexiques et de leur famille.

    Organic Chemistry--New Challenges Perhaps more than any other of the chemical subdisciplines, the scope and focus of organic chemistry has been changed by modern technology and theoretical advances, and these changes have had an equally striking effect on the undergraduate curriculum. The main challenge in organic chemistry classes has shifted, in less than a generation, from memorizing all the groups, naming conventions, and classes of reactions to understanding complex interactions at the structural and electronic orbital level. As biochemistry and polymer chemistry have grown from being the purview of a few specialists to full-blown disciplines of their own, they have also migrated from interesting, but optional, chapters at the back of the book to separate courses in the curriculum. The availability of inexpensive instrumentation means undergraduates routinely use NMR and mass spec instead of melting points to identify their products. And the changes are continuing: segments of biochemistry are metamorphizing into molecular biology and polymer chemistry is finding interconnections with materials science. In fact, as our understanding of chemical reactions at molecular and electronic levels expands, it becomes more and more difficult to decide on demarcations between the subdisciplines. Organic chemistry is an organizational construct that once was useful for segregating certain topics into a coherent two-semester introductory course. Today, it covers so much territory that no one who is an "organic chemist" can know even a small fraction of the territory and faces unique challenges when designing and teaching undergraduate courses. In a wide spectrum of articles in this issue that fall under the "organic" umbrella--from environmental chemistry to new polymer products--teachers share their specific experiences and creative solutions to these challenges, providing their colleagues with new ideas, processes, and pedagogic approaches.     Planetary exploration by astronauts will require extended periods of habitation on a planet's surface, under the influence of environmental factors that are different from those of Earth and the spacecraft that delivered the crew to the planet. Human exploration of Mars, a possible near-term planetary objective, can be considered a challenging scenario. Mission scenarios currently under consideration call for surface habitation periods of from 1 to 18 months on even the earliest expeditions. Methods: Environmental issues associated with Mars exploration have been investigated by NASA and the National Space Biomedical Research Institute (NSBRI) as part of the Bioastronautics Critical Path Roadmap Project (see http ://criticalpath.jsc.nasa.gov). Results: Arrival on Mars will immediately expose the crew to gravity only 38% of that at Earth's surface in possibly the first prolonged exposure to gravity other than the 1G of Earth's surface and the zero G of weightless space flight, with yet unknown effects on crew physiology. The radiation at Mars' surface is not well documented, although the planet's bulk and even its thin atmosphere may moderate the influx of galactic cosmic radiation and energetic protons from solar flares. Secondary radiation from activated components of the soil must also be considered. Ultrafine and larger respirable and nonrespirable particles in Martian dust introduced into the habitat after surface excursions may induce pulmonary inflammation exacerbated by the additive reactive and oxidizing nature of the dust. Stringent decontamination cannot eliminate mechanical and corrosive effects of the dust on pressure suits and exposed machinery. The biohazard potential of putative indigenous Martian microorganisms may be assessed by comparison with analog environments on Earth. Even in their absence, human microorganisms, if not properly controlled, can be a threat to the crew's health. Conclusions: Mars' surface offers a substantial challenge to the

  18. Geo Issue Tracking System

    Communication about remote sensing data quality between data providers and users as well as between the users is often difficult. The users have a hard time figuring out if a product has known problems over their region of interest and data providers have to spend a lot of effort to make this information available, if it exists. Scientific publications are one tool for communicating with the users base but they are static and mostly one way. As a data provider it is also often difficult to make feedback, received from users, available to the complete user base. The Geo Issue Tracking System (GeoITS) is an Open Source Web Application which has been developed to mitigate these problems. GeoITS combines a mapping interface (Google Maps) with a simple wiki platform. It allows users to give region specific feedback on a remote sensing product by drawing a polygon on the map and describing the problems they had using the remote sensing product in this area. These geolocated wiki entries are then viewable by other users as well as the data providers which can modify and extend the entries. In this way the conversations between the users and the data provider are no longer hidden in e.g. emails but open for all users of the dataset. This new kind of communication platform can enable better cooperation between users and data providers. It will also provide data providers with the ability to track problems their dataset might have in certain areas and resolve them with new product releases.     The issues worldwide that deserve attention pertain to international migration, the education of women, family planning, missing female children, and prevention and control of AIDS cases. Migration outside country borders has been increasing due to economic and social disparities, and uncounted and unrecognized refugees are fleeing environmental disasters. The educational status of women has increased over the past 30 years. For example, female illiteracy has declined among women aged 20-24 years from 19% to 8% in Latin America, from 38% to 12% in eastern and western Asia, and from 80% to 49% in Africa. These improvements are significant, but the problem remains, where, for instance in Africa, there is still extensive illiteracy (75% of females aged under 25 years). A 1993 Population Action International Report states that low female educational levels and weak family planning and health programs are related to large family size and high child death rates. During the 1980s family planning was linked with ecological concerns. Progress has been made in the increase in contraceptive users in developing countries from 14% to 53% during 1965-70. Fertility has declined from 6.1 children per woman in the 1950s to 3.7. Kenya is a good example of a country with high fertility that reduced fertility through its emphasis on family planning. Obstacles to increased prevalence of contraception still remain. In Mexico more than 50% of public birth control services are provided to women in secret due to fear of their husband's abuse. Population pressure can result in the loss of girl children through higher mortality, better nutrition, neglect, or gender preference in the early fetal stages. The challenge of AIDS will be in caring for the sick and adjusting to social changes due to the expected increases in orphaned children. PMID:12318864

    Advanced Instrumentation in the Educational Laboratory The instruments that chemists use in their research have changed dramatically in the past decades. The explosion in new techniques and their instrumental counterparts has been made possible by two significant advances. The rapid propagation of computer chips and circuitry provides opportunities to collect more specialized and refined data than ever before. Computer-controlled instruments can detect events that a human researcher would never perceive and can record hundreds of data points in the time that a person could only observe one or two. Coupled with these advances in technology are advances in theory that allow more sophisticated interpretation of data. A hundred years ago a sophisticated instrument was a balance that could weigh minute quantities. Today a sophisticated instrument is one that can identify the composition of that minute sample, determine its molecular weight, or reveal a great deal about its energy states and bonding. This bonanza of new instruments is wonderful for the research chemist but a curricular headache for the chemistry teacher. It takes more time to learn how to run an NMR than to use a balance, and more sophistication on the part of the student is needed to interpret the data. And yet many courses that only a generation ago sported an expensive analytical balance as its prize instrument now require students to understand and operate a whole panoply of complex tools. Teachers faced with accommodating these curricular changes will find several articles in this issue helpful--either providing information on new techniques or descriptions of how to incorporate them into the classroom.     All children, even the healthiest, have preventive and acute health care needs. Moreover, a growing number of children are chronically ill, with preventive, acute, and ongoing care needs that may be much more demanding than those for healthy children. Because children are unable to care for themselves, their parents are expected to provide a range of health care services without which the current health care system for children would not function. Under this "shadow health care system," parents or parent surrogates often need to be with the child, a requirement that can create difficulties for working parents, particularly for those whose children are chronically ill. How federal, state, and employer policies and practices mesh with the child health care needs of families is therefore a central issue in any discussion about work and family balance. In this article Mark Schuster, Paul Chung, and Katherine Vestal describe the health care needs of children; the essential health care responsibilities of parents; the perspective of employers; and the existing network of federal, state, and local family leave benefits that employed parents can access. They also identify current gaps in policies that leave unmet the needs of both parents and their employers. The authors suggest the outlines of a national family leave policy that would protect the interests of parents and employers. In essence, such a policy would build on the federal Family and Medical Leave Act, which gives some workers time off with no advance notice required and no loss of job or health insurance. But it would also include elements of California's Paid Family Leave Insurance, which expands coverage to more workers and provides partial pay during leave. Employers could be given some financial protections as well as protections against employee fraud and abuse.     Objective. Neuroprosthetics are artificial devices or systems designed to generate, restore or modulate a range of neurally mediated functions. These include sensorimotor, visual, auditory, cognitive affective and volitional functions that have been impaired or lost from congenital anomalies, traumatic brain injury, infection, amputation or neurodevelopmental and neurodegenerative disorders. Cochlear implants, visual prosthetics, deep brain stimulation, brain-computer interfaces, brain-to-brain interfaces and hippocampal prosthetics can bypass, replace or compensate for dysfunctional neural circuits, brain injury and limb loss. They can enable people with these conditions to gain or regain varying degrees of control of thought and behavior. These direct and indirect interventions in the brain raise general ethical questions about weighing the potential benefit of altering neural circuits against the potential harm from neurophysiological and psychological sequelae. Other ethical questions are more specific to the therapeutic goals of particular neuroprosthetics and the conditions for which they are indicated. These include informed consent, agency, autonomy (free will) and identity. Approach. This review is an analysis and discussion of these questions. It also includes consideration of social justice issues such as how to establish and implement fair selection criteria in providing access to neuroprosthetic research and balancing technological innovation with patients’ best interests. Main results. Neuroprosthetics can restore or improve motor and mental functions in bypassing areas of injury or modulating dysregulation in neural circuits. As enabling devices that integrate with these circuits, neuroprosthetics can restore varying degrees of autonomous agency for people affected by neurological and psychiatric disorders.     Hydrogen is often proposed as the fuel of the future, but the transformation from the present fossil fuel economy to a hydrogen economy will need the solution of numerous complex scientific and technological issues, which will require several decades to be accomplished. Hydrogen is not an alternative fuel, but an energy carrier that has to be produced by using energy, starting from hydrogen-rich compounds. Production from gasoline or natural gas does not offer any advantage over the direct use of such fuels. Production from coal by gasification techniques with capture and sequestration of CO₂ could be an interim solution. Water splitting by artificial photosynthesis, photobiological methods based on algae, and high temperatures obtained by nuclear or concentrated solar power plants are promising approaches, but still far from practical applications. In the next decades, the development of the hydrogen economy will most likely rely on water electrolysis by using enormous amounts of electric power, which in its turn has to be generated. Producing electricity by burning fossil fuels, of course, cannot be a rational solution. Hydroelectric power can give but a very modest contribution. Therefore, it will be necessary to generate large amounts of electric power by nuclear energy of by renewable energies. A hydrogen economy based on nuclear electricity would imply the construction of thousands of fission reactors, thereby magnifying all the problems related to the use of nuclear energy (e.g., safe disposal of radioactive waste, nuclear proliferation, plant decommissioning, uranium shortage). In principle, wind, photovoltaic, and concentrated solar power have the potential to produce enormous amounts of electric power, but, except for wind, such technologies are too underdeveloped and expensive to tackle such a big task in a short period of time.     Employee Assistance Program (EAP) officers, as well as personnel in other disciplines from eight NASA Centers, attended this breakout session. Ms. Brenda Blair, MA, CEAP, a guest speaker at the conference, also attended as a consultant. Representatives from the NASA Centers introduced themselves and spoke briefly about their programs. In a discussion related to the conference theme on benchmarking, quality control issues within the EAP community and adequate documentation of cases were addressed. Disposition and provision for quality assurance checks for EAP providers in single person offices were also discussed. Ms. Blair presented methods for consulting with other NASA personnel in single person EAP offices as a quality control measure. EAP intervention in critical incidents was discussed. The question of whether EAP assistance is an asset or a potential liability in those situations was addressed. Suggestions were made of topics for future EAP video-teleconference topics. A program on EAP ethics was planned for a September video teleconference. Each person was asked to provide intake forms they use to Mr. Gettleman or Ms. Blair. Ms. Blair said she would review the forms to ensure that adequate notification is provided to the client for confidentiality. She would also review them to ensure they have adequate limits of confidentiality--a topic for future video teleconferencing. Mr. Gettleman described the NASA initiative to reduce stresses in the workplace, and the activities of an ad-hoc EAP group that will make recommendations to NASA senior management. Alternative training methods were discussed for reaching target audiences such as employees at risk, supervisors, and others. Pfc. David A. Pendleton, Victim Assistance Coordinator, U.S. Capitol Police. U.S. House of Representatives made a special presentation. Pfc. Pendleton was on duty during the tragic shooting of two Federal guards at the U.S. Capitol. He related the events immediately after the incident. He

    This book is about the contribution that economics can make to the understanding of some of today's key issues. The twelve topics discussed are representative of a very wide range of concerns. The first three chapters (on nuclear deterrence, the Palestinian problem, and sanctions against South Africa) are all issues associated with international relations. Later chapters consider the environment, poverty, technology, and the role of government. Family, crime, comprehensive education and youth unemployment issues are also addressed. All of the topics included illustrate issues which are of concern to many different societies. However, it is important to examine these issues in specific contexts rather than to rely on a purely general treatment using stylised facts. In a number of chapters the issues are therefore presented in the context of the UK and sometimes particular case studies are discussed.

    Computational Chemistry for the Masses Not long ago, chemical computation was considered a specialty area requiring extensive computer knowledge, power, and time. Over the past decade, however, it has changed from the arcane pursuit of a few advanced university researchers in the area of physical chemistry to a familiar tool used by a wide range of chemists. Nevertheless, it has required its practitioners to have extensive knowledge of computer programming and a thorough understanding of theoretical chemical concepts and as a result usually was reserved for the graduate curriculum. Now a further metamorphosis is in progress, as computational chemistry moves into the undergraduate curriculum, often using off-the-shelf software--commercial packages or adaptations of them that are readily shared by their creators. As we put this issue together, we realized that many of the articles involved sophisticated computations that would not have been possible a few years ago in the courses described. Further, the hard and software used was widely available at a reasonable cost. Some of the articles focus on the teaching of computational methods and others simply incorporate it as a facet in their overall strategy; however, taken together, they reflect a strong trend to utilize a diverse set of readily available methods and products in the undergraduate curriculum. The most familiar recent use of computational chemistry is the computer design of molecules in organic, medicinal, and biochemistry. However, computational chemistry is useful for inorganic chemists as well and is now migrating to undergraduate courses. Lipkowitz, Pearl, Robertson, and Schultz (page 105) make a strong case for its inclusion and present a two-week component they have developed for their senior-level laboratory course. Comba and Zimmer (page 108) offer a review of inorganic molecular mechanics calculations, which is designed for the novice and includes the basic equations, their application to

    It would be difficult to condense current issues in nuclear reactor regulation to just a few minutes. So, let me start off by saying that I have not tried to give a comprehensive listing of issues that are currently facing the reactor program, but rather to select those that I thought were relevant as they relate to research activities. Use of probabilistic risk assessment in regulatory decisions; materials aging issues concerning steam generators and reactor vessels; high burnup fuels; accident management; and digital instrumentation and control, are just a sampling of the important issues that I want to talk about.
