  1. Le contrôle des infections au cabinet du pédiatre

    RÉSUMÉ La transmission des infections au cabinet du pédiatre est de plus en plus préoccupante. Le présent document expose les voies de transmission des infections et les principes sous-jacents aux mesures actuelles pour contrôler les infections. Pour prévenir les infections, il faut bien concevoir le cabinet et adopter des politiques administratives et de triage convenables, de même que des pratiques de base pour les soins de tous les patients (p. ex., hygiène des mains, port de gants, de masques, de lunettes de protection et d’une blouse d’hôpital pour des interventions précises; nettoyage, désinfection et stérilisation convenables des surfaces et du matériel, y compris les jouets, et techniques d’asepsie en cas d’interventions effractives) et des précautions additionnelles en cas d’infections précises. Le personnel doit avoir reçu les vaccins pertinents, et les personnes infectées doivent respecter les politiques de restriction au travail.

    Le premier cas d'infection par le VIH a été notifié en 1986 au Burkina Faso. L'ignorance qui entourait cette infection et l'absence de traitement curatif a amplifié ses conséquences sociales et économiques. La mise en œuvre des interventions communautaire est un enjeu majeur actuel qui recommande la compréhension des logiques sociales endogènes qui influencent les comportements individuels et collectifs. C'est en cela que l'implication des sciences sociales dans la lutte contre les maladies, en particulier le Sida constitue un enjeu, en termes de définition et d'identification de leur contribution. Dans notre propos, nous pouvons mettre en évidence trois niveaux de contribution des sciences sociales, notamment la sociologie, l'anthropologie, la psychologie et dans une certaine mesure le droit et l’économie, à la prévention du Sida et la prise en charge des personnes infectées par le VIH. Il faut noter que ces disciplines contribuent à la lutte contre le VIH d'une part, par des réflexions et des éléments d'analyses constructives, et d'autre part, l'aptitude des porteurs de ces disciplines est parfois sollicitée pour l'efficacité de la mise en œuvre des actions et l'organisation des systèmes de soins.   3. Prévalence du tabagisme chez le personnel de l'Hôpital Général de Douala, Cameroun

    Introduction La prévalence du tabagisme parmi le personnel de santé hospitalier au Cameroun n'est pas connue alors que le tabagisme est en expansion dans ce pays avec 13,2% de fumeurs selon l'OMS. Pour combler ce manque une enquête sur les consommations, les connaissances, opinions et attitudes vis-à-vis des fumeurs a été conduite à l'Hôpital Général de Douala, l'un des hôpitaux de référence du Cameroun. Méthodes Du 1er au 30 Avril 2010, des questionnaires anonymes ont été distribués par des enquêteurs dans les services ou via les surveillants et recueillis et analysés de façon anonyme. Résultats Sur 402 questionnaires distribués 277 ont été récupérés. La prévalence de fumeurs est de 3,6% parmi les soignants et de 9,4% parmi les autres personnels soit en moyenne sur l'ensemble de l'hôpital 5,4%. Les produits fumés étaient toujours des cigarettes. L'initiation du tabagisme à souvent été tardive (21,5 ans) et la dépendance est absente ou faible chez 33% des fumeurs. Les personnes pensent que c'est leur devoir de questionner sur le tabac et de prendre en charge les fumeurs, mais ils sont presque un sur deux à ignorer la loi Camerounaise. Conclusion Le tabagisme chez le personnel hospitalier est une réalité au Cameroun; le personnel soignant et les pouvoirs publics devraient s'impliquer davantage dans la lutte contre ce fléau qui est en expansion dans les pays du sud.   4. Evaluation de l’état vaccinal contre l’hépatite B et portage de l’Ag HBs chez le personnel médical et paramédical de l’Hôpital Central de Yaoundé, Cameroun

    Introduction L’hépatite virale B est une affection à haut risque pour le personnel de santé. Au Cameroun, la vaccination contre le virus de l’hépatite B n'est pas obligatoire pour le personnel de santé. Le but de l’étude était d'évaluer l’état vaccinal contre l’hépatite virale B et la prévalence de l’Antigène HBs au sein du personnel médical et paramédical de l’Hôpital Central de Yaoundé. Méthodes Il s'agissait d'une étude prospective, transversale menée à l’Hôpital Central de Yaoundé du 1er juillet au 31 octobre 2011. Toute personne membre du corps médical et paramédical ayant signée la fiche de consentement éclairé était incluse dans l’étude. Ceux ayant refusé de signer la fiche de consentement éclairé ou dont la fiche d'enquête n'avait pas été complètement remplie étaient exclus de l’étude. Une recherche systématique de l’antigène HBs était effectuée chez chaque personne incluse dans l’étude. Résultats Au total 282 personnes ont participé à l’étude sur 760, soit un taux de participation de 37,1%. Les personnes ne connaissant pas leur statut sérologique vis-à-vis du virus de l’hépatite B était au nombre de 221 (85%). Deux cent vingt-cinq (86,5%) personnes n'étaient pas vaccinées contre l’hépatite B. Les personnes ayant reçu moins de 3 doses du vaccin contre l’hépatite virale B étaient au nombre de 23 (8,8%). Douze personnes (4,6%) déclaraient avoir reçu les 3 doses du vaccin contre l’hépatite virale B. La prévalence de l’antigène HBs était de 6,6% (n = 12). Conclusion Cette étude montre que la prévalence de l’hépatite virale B reste élevée chez le personnel de l’Hôpital Central de Yaoundé (6,6%). Il ya un besoin de sensibilisation du personnel médical aux risques liés à l’exercice de leur profession. PMID:24778748

  5. Rachitisme patent révélé par la perte du contrôle des convulsions chez un garçon infirme moteur cérébral

    La perte du contrôle des crises chez un patient compliant au traitement est toujours source de préoccupations diagnostiques, thérapeutiques et pronostiques. Nous rapportons un cas de rachitisme chez un patient de 4 ans, infirme moteur cérébral et épileptique sous traitement par phénobarbital depuis 2 ans; rachitisme découvert à la faveur d'une perte du contrôle épileptique. Le patient était admis pour des convulsions répétées en contexte afébrile. L'observance thérapeutique était bonne, et aucune convulsion n'avait été observée pendant les 12 mois précédents. Il ne recevait pas de vitamine D. Le rachitisme était suspecté cliniquement, et confirmé par les trouvailles radiologiques et biologiques. Le contrôle des crises était retrouvé dès le 3ème jour d'hospitalisation après apports de calcium intraveineux et de vitamine D. Les convulsions étaient imputées à une hypocalcémie sur rachitisme. La prise prolongée de phénobarbital sans supplémentation en vitamine D, ainsi qu'une exposition solaire insuffisante étaient incriminées. Avant toute escalade thérapeutique, des convulsions hypocalcémiques et un rachitisme doivent toujours être exclus devant une perte du contrôle des crises chez tout patient épileptique à mobilité réduite. Par ailleurs, une supplémentation en vitamine D et une exposition suffisante au soleil devront être recommandées pour certains traitements antiépileptiques au long cours. PMID:23503922

  8. Tuberculose chez le personnel de santé du secteur public au Burundi: fréquence et facteurs de risque

    Introduction Le but de cette étude était de déterminer la fréquence de la tuberculose (TB) chez le personnel de santé du secteur public en charge des patients tuberculeux et d’évaluer les facteurs de risque de contracter la tuberculose chez ce personnel au Burundi. Méthodes Il s’agit d’une étude transversale à visée analytique réalisée auprès de 300 travailleurs prestant dans 30 centres de dépistage et de traitement de la TB (CDT) au Burundi du 16 octobre au 15 novembre 2012. Les paramètres sociodémographiques et professionnels ainsi que l’antécédent de vaccination BCG de travailleurs ayant été touché par la TB ont été analysé et comparé à ceux de travailleurs qui ne l’ont pas été. Le seuil de signification a été fixé à p < 0,05. Résultats La fréquence de la TB chez le personnel de santé est de 15%. Le risque de souffrir de la TB est de près de 4 fois chez les travailleurs âgés d’au moins 50 ans (OR=3,73; 1,53-9,08), chez ceux qui n’ont jamais reçu de vaccin de BCG (OR=3,73; 1,24-11,03), chez ceux qui n’ont pas de cicatrice vaccinale de BCG (OR=3,80; 1,67-8,62) et chez ceux qui travaillent depuis au moins 6 ans dans un CDT (OR=3,79; 1,44-9,96); ce risque est de 9 fois chez ceux qui sont mariés (OR=9,42; 1,26-70,23), de 8 fois chez ceux qui n’aèrent pas leurs salles de travail (OR=8,20; 1,48-48,23) et de 6 fois chez ceux qui ont comme profession nettoyeur ou aide-soignant (OR=6,12; 2,92-12,82). Par contre, aucune corrélation statistiquement significative n’a été observée entre le fait de souffrir de la TB et le sexe mais aussi le nombre d’heures de contact d’un travailleur avec un patient tuberculeux (p>0,05). Conclusion L’âge, l’antécédent de vaccination de BCG ainsi que la majorité de paramètres professionnels sont en association avec la maladie TB des travailleurs de CDT.     Introduction Véritable attribut de la féminité, le port de chaussures à talons est devenu une exigence professionnelle dans certaines structures des pays en développement comme le Bénin. L'objectif de ce travail est de déterminer les effets spécifiques du port de chaussures à talons hauts sur le squelette appendiculaire inférieur. Méthodes Des examens radiographiques de face et de profil de la cheville et du genou ont été effectués sur 122 femmes volontaires, âgées en moyenne de 25,09 ± 1,34 ans et ayant les genu varum qui ont participé à cette étude. Résultats Les résultats ont indiqué une augmentation significative de l’écart inter malléolaire, de l’écart inter condylien, de l'angle antérieur du pied et de l'angle fémoro-patellaire. Une diminution significative de l'angle postérieur du pied a été constatée. Conclusion Le port de chaussures à talons hauts est l'une des causes de la gonarthrose, des troubles des articulations de la cheville et du pied. PMID:26113922

  10. Measurements of daily urinary uranium excretion in German peacekeeping personnel and residents of the Kosovo region to assess potential intakes of depleted uranium (DU).

    Following the end of the Kosovo conflict, in June 1999, a study was instigated to evaluate whether there was a cause for concern of health risk from depleted uranium (DU) to German peacekeeping personnel serving in the Balkans. In addition, the investigations were extended to residents of Kosovo and southern Serbia, who lived in areas where DU ammunitions were deployed. In order to assess a possible DU intake, both the urinary uranium excretion of volunteer residents and water samples were collected and analysed using inductively coupled plasma-mass spectrometry (ICP-MS). More than 1300 urine samples from peacekeeping personnel and unexposed controls of different genders and age were analysed to determine uranium excretion parameters. The urine measurements for 113 unexposed subjects revealed a daily uranium excretion rate with a geometric mean of 13.9 ng/d (geometric standard deviation (GSD)=2.17). The analysis of 1228 urine samples from the peacekeeping personnel resulted in a geometric mean of 12.8 ng/d (GSD=2.60). It follows that both unexposed controls and peacekeeping personnel excreted similar amounts of uranium. Inter-subject variation in uranium excretion was high and no significant age-specific differences were found. The second part of the study monitored 24 h urine samples provided by selected residents of Kosovo and adjacent regions of Serbia compared to controls from Munich, Germany. Total uranium and isotope ratios were measured in order to determine DU content. (235)U/(238)U ratios were within +/-0.3% of the natural value, and (236)U/(238)U was less than 2 x 10(-7), indicating no significant DU in any of the urine samples provided, despite total uranium excretion being relatively high in some cases. Measurements of ground and tap water samples from regions where DU munitions were deployed did not show any contamination with DU, except in one sample.   11. Effets du stress familial lié au diabète sur le contrôle glycémique chez les jeunes diabétiques de type 1

    Objectif Examiner la façon dont le stress familial influence le contrôle glycémique chez les patients diabétiques de moins de 18 ans. Sources des données Une recherche des études pertinentes publiées depuis 1990 a été réalisée dans PubMed et Scopus à l'aide des mots-clés suivants en anglais: diabetes type 1, glycemic control, family stress, family conflict et family function. Sélection des études La recension initiale a permis de cerner un total de 1 478 articles. La synthèse finale portrait sur 6 études de cohortes, 3 études transversales et 1 étude qualitative dans lesquelles le stress familial était évalué à l'aide d'instruments de mesure des conflits spécifiquement reliés au diabète et le contrôle glycémique était mesuré en fonction de l'hémoglobine glycosylée. Synthèse Dans la plupart des études, il existait une corrélation négative entre le stress familial et le contrôle glycémique des patients. Le bon fonctionnement familial était fortement relié à un bon contrôle glycémique des patients, tandis que les conflits familiaux étaient associés à un mauvais contrôle glycémique. Les familles dont la situation socioéconomique était défavorisée, celles ayant des adolescents atteints de diabète et les familles monoparentales étaient plus enclines à vivre un stress relié au diabète et étaient, par conséquent, plus susceptibles de connaître un moins bon contrôle glycémique. Conclusion Les interventions psychologiques thérapeutiques et les programmes éducatifs peuvent aider à atténuer le stress familial relié au diabète et amélioreront probablement le contrôle glycémique.

    Résumé Objectif Répondre aux questions fréquentes concernant l’utilisation du nouveau vaccin contre l’herpès zoster (HZ). Sources de l’information Les résultats publiés d’études cliniques et d’autres études, les recommandations du Comité consultatif national de l’immunisation du Canada et de l’Advisory Committee on Immunization Practices des États-Unis; des données ont aussi été obtenues dans la monographie du produit approuvée par Santé Canada sur le vaccin. Principal message L’herpès zoster est causé par la réactivation du virus varicelle-zoster; l’algie post-zostérienne (APZ) est sa complication la plus fréquente et la plus grave. L’incidence de l’APZ après un zona est directement reliée à l’âge, notamment 50 % des personnes affectées de plus de 60 ans qui ont des douleurs persistantes non soulagées. Le vaccin à base de virus vivant contre le HZ réduit l’incidence du zona d’environ 50 % et de 2 tiers l’apparition des APZ. De plus, les symptômes sont atténués et de plus courte durée chez les personnes vaccinées. Le vaccin est contre-indiqué chez de nombreux patients immunodéprimés et pourrait ne pas être efficace chez les patients qui prennent des médicaments antiviraux qui agissent contre le virus HZ. Les médecins devraient être au courant des différentes recommandations touchant ces groupes. Conclusion Le vaccin contre le HZ est une mesure de prévention sécuritaire et efficace pour réduire le fardeau global et la gravité du zona chez les adultes plus âgés. Le vaccin semble rentable lorsqu’il est administré à des adultes de 60 ans et plus.

  13. Mise à jour sur le nouveau vaccin 9-valent pour la prévention du virus du papillome humain

    Résumé Objectif Informer les médecins de famille quant à l’efficacité, à l’innocuité, aux effets sur la santé publique et à la rentabilité du vaccin 9-valent contre le virus du papillome humain (VPH). Qualité des données Des articles pertinents publiés dans PubMed jusqu’en mai 2015 ont été examinés et analysés. La plupart des données citées sont de niveau I (essais randomisés et contrôlés et méta-analyses) ou de niveau II (études transversales, cas-témoins et épidémiologiques). Des rapports et recommandations du gouvernement sont aussi cités en référence. Message principal Le vaccin 9-valent contre le VPH, qui offre une protection contre les types 6, 11, 16, 18, 31, 33, 45, 52 et 58 du VPH, est sûr et efficace et réduira encore plus l’incidence des infections à VPH, de même que les cas de cancer lié au VPH. Il peut également protéger indirectement les personnes non immunisées par l’entremise du phénomène d’immunité collective. Un programme d’immunisation efficace peut prévenir la plupart des cancers du col de l’utérus. Les analyses montrent que la rentabilité du vaccin 9-valent chez les femmes est comparable à celle du vaccin quadrivalent original contre le VPH (qui protège contre les types 6, 11, 16 et 18 du VPH) en usage à l’heure actuelle. Toutefois, il faut investiguer plus en profondeur l’utilité d’immuniser les garçons avec le vaccin 9-valent contre le VPH. Conclusion en plus d’être sûr, le vaccin 9-valent protège mieux contre le VPH que le vaccin quadrivalent. Une analyse coûtefficacité en favorise l’emploi, du moins chez les adolescentes. Ainsi, les médecins devraient recommander le vaccin 9-valent à leurs patients plutôt que le vaccin quadrivalent contre le VPH.

    A personnel dosimeter sensitive to both gamma and beta radiation is described. The dosimeter consists of an electrical conductive cylinder having a wall thickness of substantially 7 milligrams per square centimeter and an electrode disposed axially within the cylinder and insulated therefrom to maintain a potential impressed between the electrode and the cylinder. A cylindrical perforated shield provided with a known percentage of void area is disposed concentrically about the cylinder. The shield is formed of a material which does not contain more than 15 percent of an element higher than atomic weight 13. The dose actually received is at most the gamma dose plus the beta dose indicated by discharge of the dosimeter divided by the known percentage.

  18. Modelisation numerique et validation experimentale d'un systeme de protection contre le givre par elements piezoelectriques

    Le degivrage au moyen d'actuateurs piezoelectriques est considere comme une avenue prometteuse pour le developpement de systemes a faible consommation d'energie applicables aux helicopteres legers. Ce type de systeme excite des frequences de resonances d'une structure pour produire des deformations suffisantes pour rompre l'adherence de la glace. Par contre, la conception de tel systeme demeure generalement mal comprise. Ce projet de maitrise etudie l'utilisation de methodes numeriques pour assister la conception des systemes de protection contre le givre a base d'elements piezoelectriques. La methodologie retenue pour ce projet a ete de modeliser differentes structures simples et de simuler l'excitation harmonique des frequences de resonance au moyen d'actuateurs piezoelectriques. Le calcul des frequences de resonances ainsi que la simulation de leur excitation a ensuite ete validee a l'aide de montages experimentaux. La procedure a ete realisee pour une poutre en porte-a-faux et pour une plaque plane a l'aide du logiciel de calcul par elements finis, Abaqus. De plus, le modele de la plaque plane a ete utilise afin de realiser une etude parametrique portant sur le positionnement des actuateurs, l'effet de la rigidite ainsi que de l'epaisseur de la plaque. Finalement, la plaque plane a ete degivree en chambre climatique. Des cas de degivrage ont ete simules numeriquement afin d'etudier la possibilite d'utiliser un critere base sur la deformation pour predire le succes du systeme. La validation experimentale a confirme la capacite du logiciel a calculer precisement a la fois les frequences et les modes de resonance d'une structure et a simuler leur excitation par des actuateurs piezoelectriques. L'etude revele que la definition de l'amortissement dans le modele numerique est essentiel pour l'obtention de resultats precis. Les resultats de l'etude parametrique ont demontre l'importance de minimiser l'epaisseur et la rigidite afin de reduire la valeur des frequences

Il s'agit d'une étude transversale réalisée dans quatre institutions hospitalières de la province du Katanga dans des districts sanitaires différents (Hôpital Jason Sendwe à Lubumbashi, Hôpital Gécamines Panda à Likasi, Hôpital Gécamines du personnel à Kolwezi, Hôpital General de référence de Kamina) et qui a porté sur 154 cas de fentes labiopalatines enregistrés au cours de la période allant du 1er mai 2010 au 30 septembre 2012. L'âge moyen de consultation était de 11,8 ans et une prédominance masculine (55,2%) était notée. Un pic était noté chez les deux premiers nés de la famille (55,8%). Nous avons enregistré 20,7% des cas de consanguinité dont 54,2% de premier degré. La fréquence des différents types de fentes labiopalatines diminue au fur à mesure que la fente s'étend de la lèvre supérieure au palais en passant par l'alvéole : 72% (labiales), 21,4% (labiopalatines) et 7,7% (palatines). Les variétés unilatérales sont plus fréquentes (76,7%) que les bilatérales (16,1%). Dans les formes unilatérales, le côté gauche est plus concerné (47,1%) par rapport au côté droit (38,6%). L'évaluation de la gravité selon Anastassov montre que 50,6% de nos patients étaient de degré moyen et 16,2% étaient à un degré sévère. Les malformations associées ont été retrouvées dans 5% des cas et elles sont à prédominance squelettique. C'est la technique de Millard qui a été la plus pratiquée (72/130). Les résultats étaient excellents dans 71,5% contre 1,5% de mauvais. Le séjour d'hospitalisation était de 3 jours et le taux de complications post opératoire était de 2,98%. PMID:25328615

    Le ballon de contre pulsion intra-aortique (BCPIA) est fréquemment utilisé en chirurgie cardiaque, comme moyen d'assistance circulatoire en cas de bas débit cardiaque. Il est d'intérêt clinique de déterminer les facteurs pronostiques chez les patients porteurs d'un BCPIA en chirurgie cardiaque, et qui restent un sujet rarement élucidé dans la littérature. L'objectif de notre travail est de déterminer les facteurs prédictifs de morbi-mortalité chez les patients sous ballon de contre pulsion intraortique en périopératoire d'une chirurgie cardiaque. Il s'agit d'une étude rétrospective portant sur l'ensemble des patients opérés en chirurgie cardiaque sous circulation extracorporelle, et ayant bénéficiés de la mise en place d'un ballon de contre pulsion intra-aortique en périopératoire, au service de chirurgie cardiovasculaire de l'Hôpital Militaire Mohamed V de Rabat, entre le mois de janvier 2005 et le mois d'aout 20014. Soixante dix patients ont été inclus dans notre étude. En analyse univariée l'âge, la dyspnée de stade III et IV, l'insuffisance cardiaque, la présence d'un infarctus du myocarde, d'une coronaropathie mono et bitronculaire, les anomalies du doppler de trons supra-aortique et du membre inférieur, le caractère urgent de la chirurgie, la durée de la circulation extracorporelle, l'instabilité hémodynamique postopératoire, le saignement et l'insuffisance rénale postopératoire étaient statistiquement associés à une mortalité postopératoire élevée. La dyskinésie préopératoire et la sortie de circulation extracorporelle sous drogues étaient associées à une morbidité globale élevée. En analyse multi variée, seule l'âge, constituait un facteur de risque indépendant de mortalité dans notre série avec un Odds Ratio (OR): 1,89 ; un Intervalle de Confiance (IC) 95% de (1,52-4,97) et un p =0,045.     Résumé Question Compte tenu du taux à la hausse de la coqueluche chez les enfants, plusieurs familles m’ont demandé quels moyens prendre pour protéger leurs tout-petits de cette infection. Quelles devraient être mes recommandations à ces familles? Réponse La coqueluche est une maladie évitable qui est endémique dans le monde entier. Chez les adultes, la coqueluche cause une maladie bénigne semblable à un rhume, suivie d’une toux persistante. Chez les nourrissons, elle peut causer de l’apnée, des convulsions, une encéphalopathie, une bronchopneumonie et la mort. Les décès dus à la coqueluche se produisent dans 86 % des cas chez des nourrissons de moins de 4 mois. La stratégie du cocooning, c’est-à-dire la vaccination des adultes en étroit contact avec des nourrissons, est recommandée par de nombreuses agences mondiales et nationales, mais elle ne prévient probablement que 20 % des cas de coqueluche chez les nourrissons. La vaccination durant la grossesse est plus efficace, mais elle n’est pas encore approuvée au Canada. Il n’a pas été démontré que la vaccination à la naissance soit uniformément efficace et elle n’est donc pas recommandée à l’heure actuelle.

Inhalation intake rates and associated dose rates were estimated from cascade impactors worn by sample recovery personnel and from cascade impactors that served as area monitors. Ingestion intake rates and associated dose rates were estimated from cotton gloves worn by sample recovery personnel and from wipe-tests samples from the interior of vehicles perforated with large-caliber DU munitions. The mean DU inhalation intake rate for level II personnel ranged from 0.447 mg h(-1) based on breathing zone monitor data (in and around a perforated vehicle) to 14.5 mg h(-1) based on area monitor data (in a perforated vehicle). The mean DU ingestion intake rate for level II ranged from 4.8 mg h(-1) to 38.9 mg h(-1) based on the wipe-tests data including surface-to-glove transfer factors derived from the Capstone data. Based on glove contamination data, the mean DU ingestion intake rates for level II and level III personnel were 10.6 mg h(-1) and 1.78 mg h(-1), respectively. Effective dose rates and peak kidney uranium concentration rates were calculated based on the intake rates. The peak kidney uranium concentration rate cannot be multiplied by the total exposure duration when multiple intakes occur because uranium will clear from the kidney between the exposures.     A description is given of a personnel neutron dosimeter capable of indicating the complete spectrum of the neutron dose received as well as the dose for each neutron energy range therein. The device consists of three sets of indium foils supported in an aluminum case. The first set consists of three foils of indium, the second set consists of a similar set of indium foils sandwiched between layers of cadmium, whereas the third set is similar to the second set but is sandwiched between layers of polyethylene. By analysis of all the foils the neutron spectrum and the total dose from neutrons of all energy levels can be ascertained.

    According to the current literature, the term "du Bois sign" characterizes the condition of a shortened fifth finger as a symptom of congenital syphilis, Down syndrome, dyscrania, and encephalic malformation. Modern medical dictionaries and text books attribute the eponym to the French gynecologist Paul Dubois (1795-1871). Yet, a literature analysis revealed incorrect references to the person and unclear definitions of the term. Our findings showed that the origin of the term is based on observations made by the Swiss dermatologist Charles du Bois (1874-1947) in connection with congenital syphilis. In addition, a further eponymical fifth finger sign is closely associated with the du Bois sign. In conclusion, the du Bois sign has only limited diagnostic value and is frequently occurring in the normal healthy population. PMID:21263293

    Introduction Dans notre milieu, il n'existe aucune politique de prévention du Retard de Croissance Intra-Utérin (RCIU) clairement défini. L'objectif de ce travail était d'identifier les facteurs de risque de RCIU afin de proposer une stratégie de lutte contre cette pathologie en agissant surtout sur des facteurs pouvant faire l'objet d'une action préventive. Méthodes Une étude cas-témoins a été menée dans 11 centres hospitaliers de Lubumbashi en République Démocratique du Congo, de Janvier 2010 à Juin 2011, dans le but d'identifier les facteurs de risque du retard de croissance intra-utérin (RCIU). Au total 420 gestantes (cas et témoins) avec grossesse monofoetale d'au moins 24 semaines d'aménorrhée ont été inclues dans l'étude. Les cas correspondaient aux gestantes dont le poids du fœtus était resté inférieur au 10 eme percentile des courbes de référence d'Alexander, après 2 échographies successives réalisées à intervalle de 4 semaines. Les témoins correspondaient aux gestantes dont le poids du fœtus était supérieur ou égal au 10 eme percentile de mêmes courbes. A chaque cas a été apparié un témoin de même parité porteur d'une grossesse de même âge. Résultats L'analyse univariée a identifié comme facteurs de risque: la taille maternelle. Conclusion L'amélioration du niveau socio-économique des populations, la lutte contre le paludisme et les consultations prénatales mieux organisées couplées à une meilleure éducation sanitaire et nutritionnelle peuvent contribuer sensiblement à la réduction de la fréquence du RCIU à Lubumbashi.     Résumé Objectif Explorer les raisons pour lesquelles la perte de poids à long terme échoue la plupart du temps et évaluer les conséquences de diverses trajectoires pondérales, y compris la stabilité, la perte et le gain. Source des données Les études qui évaluent les paramètres pondéraux dans la population sont en majorité observationnelles. Des données probantes de niveau I ont été publiées pour évaluer l’influence des interventions relatives au poids sur la mortalité et la qualité de vie. Message principal Seulement un petit pourcentage des personnes qui désirent perdre du poids réussissent à le faire de manière durable. La mortalité est la plus faible chez les personnes se situant dans la catégorie de poids élevé-normal et surpoids. La trajectoire pondérale la plus sécuritaire est la stabilité du poids avec une optimisation de la condition physique et métabolique. Il est démontré que la mortalité est plus faible chez les personnes ayant des comorbidités reliées à l’obésité si elles perdent du poids. Il est aussi établi que la qualité de vie sur le plan de la santé est meilleure chez les personnes obèses qui perdent du poids. Par contre, la perte de poids chez une personne obèse autrement en santé est associée à une mortalité accrue. Conclusion La perte de poids est recommandable seulement chez les personnes qui ont des comorbidités reliées à l’obésité. Les personnes obèses en santé qui veulent perdre du poids devraient être informées qu’il peut y avoir des risques à le faire. Une stratégie qui se traduit par un indice de masse corporelle stable avec une condition physique et métabolique optimisée, peu importe le poids, est l’option d’intervention la plus sécuritaire en ce qui concerne le poids.

De plus, quand on réfléchit à la pertinence de prescrire des opioïdes et aux nombreuses implications sur le plan de l’éthique, il est utile de fonder sa décision sur l’application des principes éthiques de la bienfaisance, de la non-malfaisance, du respect de l’autonomie et de la justice. En outre, il est essentiel de se tenir au fait des changements dans les lois et la règlementation, ainsi qu’à propos des registres électroniques provinciaux des ordonnances d’opioïdes. Conclusion Les médecins doivent s’assurer que la douleur de leurs patients est bien évaluée et prise en charge. Le contrôle optimal de la douleur peut exiger la prescription d’opioïdes. Cependant, l’obligation de soulager la douleur doit être exercée en juste équilibre avec l’acquittement de la responsabilité tout aussi importante de ne pas exposer les patients au risque de dépendance et de ne pas créer de possibilités de détournement de drogues, de trafic et de dépendance chez d’autres personnes.     An investigation was performed to assess a possible health risk of depleted uranium (DU) for residents and German peacekeeping personnel serving on the Balkans. In order to evaluate a possible DU intake, the urinary uranium excretions of volunteers were collected and analysed using inductively coupled plasma mass spectrometry (ICP-MS). In total, more than 1300 urine samples from soldiers, civil servants and unexposed controls of different genders and ages were analysed to determine uranium excretion parameters. All participating volunteers, aged 3-92 y, were grouped according to their gender and age for evaluation. The results of the investigation revealed no significant difference between the unexposed controls and the peacekeeping personnel. In addition, the geometric means of the daily urinary excretion in peacekeeping personnel, ranging from 3 to 23 ng d(-1) for different age groups, fall toward the lower end of renal uranium excretion values published for unexposed populations in literature. The measured data were compared with the International Commission on Radiological Protection prediction for the intake of natural uranium by unexposed members of the public. The two data sets are in good agreement, indicating that no relevant intake of additional uranium, either natural or DU, has appeared for German peacekeeping personnel serving on the Balkans.     The offset printing process rests primarily on the balance between ink and damping solution. However, this balance remains difficult to realize. The corresponding wastes of time and into consumable (ink, paper and damping solution) are harmful for the industrialist, the consumer and the environment. The main feature of this work is to control and thus to optimize these flux. With this intention, two optical sensors functioning in reflection were developed. A laser with receiving photodiode for the control of damping solution film thickness, and an optical fiber related to a photoelectric sensor for control of ink film thickness. The current study aims at the calibration of these two sensors for respectively the control of water and ink flux in the offset press. Le procédé d'impression offset repose essentiellement sur l'équilibre entre la solution de mouillage et l'encre. Toutefois, cet équilibre reste une tâche difficile à réaliser. Les pertes en temps et en consommables (encre, papier, solution de mouillage) correspondantes sont nuisibles à la fois pour l'industriel, le consommateur et l'environnement. L'objectif de ce travail est de contrôler et donc d'optimiser ces flux pour un tirage donné. Pour ce faire, deux capteurs de type optique fonctionnant en réflexion ont été développés. Un laser avec photodiode réceptrice pour le contrôle du film de solution de mouillage, et une fibre optique liée à un capteur photoélectrique pour le contrôle du film d'encre. L'étude actuelle vise la mise en ouvre et l'étalonnage de ces deux capteurs pour la mesure et le contrôle des films respectifs d'eau et d'encre dans la presse offset.

The Detailer's Assistant expert system is being developed to improve the detailers' ability to satisfy the needs of their constituents and Navy management. An initial prototype of the Detailer's Assistant is now being evaluated. Numerous upgrades and extensions should lead to an operational system in the near future. Further development to a production system will involve additional research in machine learning, intelligent database methods, and cooperating expert systems.

    Introduction Le paludisme est une maladie parasitaire à transmission vectorielle qui constitue un problème majeur de santé publique dans les pays tropicaux, particulièrement les pays de l'Afrique Subsaharienne. La présente étude avait pour objectif d'identifier le niveau d'utilisation de la MILD chez les enfants de moins de 5 ans dans la zone de santé de Mumbunda. Méthodes L’étude, de type transversal, a été mené du 25 au 27 octobre 2013 et a porté sur un échantillon de 410 ménages tirés au hasard et ayant au moins un enfant de moins de 5 ans. Résultats Parmi les ménages qui avaient déclaré possédaient la MILD 13,1% (n=54) des répondant avaient déclaré les utilisé chez les enfants de moins de 5ans et 80,2% pour tout le monde. Dans 22,0% des cas, les répondants ont évoqué le manque d'argent comme motif de la non possession de la MILD. Les répondants savaient déclarer à 79,8% avoir utilisé la MILD pour se protéger contre la malaria et à 66,3% pour se protéger contre les piqures des moustiques. La MILD a été étalée à l'ombre pendant 24h avant d’être utilisée pour la première fois par 77,9% des ménages. Les répondants avaient déclaré à 15,3% (n=63) avoir reçu les conseils par les médias pour l'usage de la moustiquaire. Et le personnel médical était la source la plus importante pour expliquer le mode d'emploi de la MILD pour 51,2% des ménages. Conclusion La réussite pour faire reculer le paludisme doit nécessairement passer par la prévention, le suivi et l’évaluation de l'utilisation des moustiquaires imprégnées au niveau de la zone de santé de Mumbunda.     Adjusting an experimental device of control by eddy-currents in sinusoidal or pulse mode requires the optimization of a certain number of parameters. Generally, the implementation of the latter operations is long and delicate, since it is issued of many compromises. The simulation study presented in this paper can be considered as a qualitative help because it specifies the the behaviour of the different parameters implied in the adjustment of the experimental defect reserach. First, we prove the influence of the lift-off on the defect detention's sensitivity of the flat sensor we used. Next we give prominence to the fact, in practice, we cannot disregard the geometry of the sensor, as we could do in theory. This result will lead us to define a family of curves showing the evolution of the depth of penetration as a function of the pulse's duration for different coil radii. We also determine the maximal sensor's sensitivity area, with the aim of locating the defect detected in the plate with precision. Finally, we prove the dependency between the true value of the depth of penetration and the thickness of the considered metallic plate. This very important result should equally be taken under consideration in the case of a real control.     The Radiation Security System (RSS) at the Los Alamos Neutron Science Center (LANSCE) provides personnel protection from prompt radiation due to accelerated beam. The Personnel Access Control System (PACS) is a component of the RSS that is designed to prevent personnel access to areas where prompt radiation is a hazard. PACS was designed to replace several older personnel safety systems (PSS) with a single modem unified design. Lessons learned from the operation over the last 20 years were incorporated into a redundant sensor, single-point failure safe, fault tolerant, and tamper-resistant system that prevents access to the beam areas by controlling the access keys and beam stoppers. PACS uses a layered philosophy to the physical and electronic design. The most critical assemblies are battery backed up, relay logic circuits; less critical devices use Programmable Logic Controllers (PLCs) for timing functions and communications. Outside reviewers have reviewed the operational safety of the design.     The fourth Personnel Dosimetry Intercomparison Study was held at the Oak Ridge National Laboratory's Dosimetry Applications Research Facility during March 15-23, 1978. The Health Physics Research Reactor (HPRR) used unshielded, with a 12-cm-thick Lucite shield, a 20-cm-thick concrete shield, or a 5-cm-thick steel and 15-cm-thick concrete shield, and provided four neutron and gamma-ray spectra. Then the dose was calculated based on the HPRR neutron spectra and dose conversion factors which had been determined previously for the four spectra. The results of these personnel dosimetry intercomparison studies reveal that estimates of dose equivalent vary over a wide range. The standard deviation of the mean of participants data for gamma measurements was in the range of 29 to 43%; for neutrons it was 57 to 188%. (PCS)

    The fifth Personnel Dosimetry Intercomparison Study (PDIS) was conducted at the Oak Ridge National Laboratory's (ORNL) Dosimetry Applications Research (DOSAR) facility on March 20-22, 1979. This study is the latest PDIS in the continuing series started at the DOSAR facility in 1974. The PDIS is a three day study, typically in March, where personnel dosimeters are mailed to the DOSAR facility, exposed to a range of low-level neutron radiation doses (1 to 15 mSv or equivalently, 100 to 1500 mrem) and neutron-to-gamma ratios (1:1-10:1) using the Health Physics Research Reactor (HPRR) as the radiation source, and returned to the participants for evaluation. This report is a summary and analysis of the results reported by the various participants. The participants are able to intercompare their results with those of others who made dose measurements under identical experimental conditions.

    Résumé Objectif Déterminer si le yoga thérapeutique améliore la qualité de vie de patients atteints de cancer. Sources des données Recherche effectuée avec la base de données MEDLINE (1950–2010) en utilisant les mots-clés yoga, cancer et quality of life. Sélection des études Priorité accordée aux études cliniques randomisées contrôlées évaluant l’effet du yoga sur différents symptômes susceptibles de se présenter chez des patients atteints de cancer en Amérique du Nord. Synthèse Quatre études cliniques randomisées contrôlées ont d’abord été analysées, puis 2 études sans groupe-contrôle. Trois études réalisées en Inde et au Proche-Orient ont également apporté des éléments intéressants au plan méthodologique. Les interventions proposées comprenaient des séances de yoga d’une durée et d’une fréquence variables. Les paramètres mesurés variaient également d’une étude à l’autre. Plusieurs symptômes ont connu des améliorations significatives avec le yoga (meilleure qualité du sommeil, diminution des symptômes anxieux ou dépressifs, amélioration du bien-être spirituel, etc.). Il a aussi semblé que la qualité de vie, dans sa globalité ou dans certaines de ses composantes spécifiques, s’améliorait. Conclusion La variété des effets bénéfiques produits, l’absence d’effet secondaire et le rapport coût-bénéfice avantageux du yoga thérapeutique en fait une intervention intéressante à suggérer par les médecins de famille aux patients atteints de cancer. Certaines lacunes méthodologiques ont pu diminuer la puissance statistique des études présentées, à commencer par la taille restreinte des échantillons et par l’assiduité variable des patients soumis à l’intervention.     Concern has been expressed in paediatric radiology regarding the magnitude of the extremity dose received by attending personnel during routine fluoroscopic procedures and CT. Common procedures that may be of short duration in adults can be quite the opposite in paediatric patients. The extremities of attending personnel are more likely to be exposed to the primary beam and for a longer period of time owing to a variety of reasons such as assisting in the procedure or physically restraining the patient during the examination. During the period mid 1998 to mid 2000, two paediatric radiologists, four senior radiographers and two paediatric nurses were monitored using ring thermoluminescent dosemeters (TLDs). Each participant wore the ring TLD on either the left or right ring finger, depending on which hand the individual favoured. Left/right asymmetrical studies were not conducted, nor were records kept of whether an examination used a grid or gridless technique. Initial apprehension about higher paediatric fluoroscopic and CT extremity doses was dispelled as a result of this quantitative dosimetric study. PMID:11932219

    The Dosimetry Applications Research (DOSAR) Group at the Oak Ridge National Laboratory (ORNL) has conducted sixteen Neutron Personnel Dosimetry Intercomparison Studies (PDIS) since 1974. During these studies dosimeters are mailed to DOSAR, exposed to low-level (typically in the 0.3 -- 5.0 mSv range) neutron dose equivalents in a variety of mixed neutron-gamma radiation fields, and then returned to the participants for evaluation. The Health Physics Research Reactor (HPRR) was used as the primary radiation source in PDIS 1--12 and radioisotopic neutron sources at DOSAR's Radiation Calibration Laboratory (RADCAL) were mainly used, along with sources and accelerators at cooperating institutions, in PDIS 13--16. Conclusions based on 13,560 measurements made by 146 different participating organizations (102 - US) are presented.

    The Cri du Chat syndrome (CdCS) is a genetic disease resulting from a deletion of variable size occurring on the short arm of chromosome 5 (5p-). The incidence ranges from 1:15,000 to 1:50,000 live-born infants. The main clinical features are a high-pitched monochromatic cry, microcephaly, broad nasal bridge, epicanthal folds, micrognathia, abnormal dermatoglyphics, and severe psychomotor and mental retardation. Malformations, although not very frequent, may be present: cardiac, neurological and renal abnormalities, preauricular tags, syndactyly, hypospadias, and cryptorchidism. Molecular cytogenetic analysis has allowed a cytogenetic and phenotypic map of 5p to be defined, even if results from the studies reported up to now are not completely in agreement. Genotype-phenotype correlation studies showed a clinical and cytogenetic variability. The identification of phenotypic subsets associated with a specific size and type of deletion is of diagnostic and prognostic relevance. Specific growth and psychomotor development charts have been established. Two genes, Semaphorin F (SEMAF) and δ-catenin (CTNND2), which have been mapped to the "critical regions", are potentially involved in cerebral development and their deletion may be associated with mental retardation in CdCS patients. Deletion of the telomerase reverse transcriptase (hTERT) gene, localised to 5p15.33, could contribute to the phenotypic changes in CdCS. The critical regions were recently refined by using array comparative genomic hybridisation. The cat-like cry critical region was further narrowed using quantitative polymerase chain reaction (PCR) and three candidate genes were characterised in this region. The diagnosis is based on typical clinical manifestations. Karyotype analysis and, in doubtful cases, FISH analysis will confirm the diagnosis.     The moisture barrier membrane used in the manufacturing of fire protective clothing has an important protective role during use in service. This membrane usually consists of a coating of expanded polytetrafluoroethylene (e-PTFE) laminated to a Nomex® fabric. This study has examined the effects of thermal, photochemical and hydrolytic aging on the performance of this membrane. The properties of this material may evolve over time depending on the conditions of use in service. Thermal aging were conducted at five temperatures: 190, 220, 275, 300 and 320°C. Four UV intensities (0,35, 0,68, 1,35 and 1 W/m2) and three temperatures (50, 70 and 80°C) were used for the accelerated photochemical aging and three moisture levels were selected (60, 80 and 95 %) for the hydrolytic aging. The measured properties include tensile strength, tear strength and water vapor permeability. Complementary physico-chemical and morphological analysis were also performed. Significant reductions in the mechanical properties of the membrane after thermal, photochemical, hydrolytic, and alternated aging have been observed. They can be attributed among others to a degradation of the Nomex® fibers. The effect of thermal aging on the mechanical properties of the membrane can be described by an Arrhenius law. Other models have been developed to predict the service lifetime for the photochemical aging and the synergistic effect of these aging agents. Concerning the water vapour permeability, two competitive mechanisms have been observed. On the one hand a closure of the micropores in the e-PTFE membrane was observed at the lowest aging temperatures, leading to a decrease in the permeability. On the other hand, cracks appeared in the membrane for the highest aging temperatures and/or for sufficiently long exposure times. These cracks produce a dramatic increase in the permeability, far beyond the value observed in the new material. Measurements by infrared spectroscopy (FTIR), X-ray diffraction (XRD), differential scanning calorimetry (DSC), scanning electron microscopy (SEM) and by atomic force microscopy (AFM) revealed changes in the physicochemical structure of both components of the membrane after aging treatments. These results allow a better understanding of the phenomena occurring during aging of high-performance microporous laminates. They also provide tools to help improve the safety of fire protective clothing. From the experimental data obtained during this study and using a lifetime criterion corresponding to a decrease of 50% of the initial tearing force, a mathematical model has been used to predict the lifetime of the e-PTFE / Nomex® membrane during its aging in service. These models take into account the effects of thermal, photochemical and hydrolytic aging, as well as it's synergistic effects.     The Personnel Occupied Woven Envelope Robot (POWER) provides an alternative to extravehicular activity (EVA) of space suited astronauts and/or use of long slender manipulator arms such as are used in the Shuttle Remote Manipulator System. POWER provides the capability for a shirt sleeved astronaut to perform such work by entering a control pod through air locks at both ends of an inflated flexible bellows (access tunnel). The exoskeleton of the tunnel is a series of six degrees of freedom (Six-DOF) articulated links compressible to 1/6 of their fully extended length. The operator can maneuver the control pod to almost any location within about 50 m of the base attachment to the space station. POWER can be envisioned as a series of hollow Six-DOF manipulator segments or arms wherein each arm grasps the shoulder of the next arm. Inside the hollow arms ia a bellow-type access tunnel. The control pod is the fist of the series of linked hollow arms. The fingers of the fist are conventional manipulator arms under direct visual control of the nearby operator in the pod. The applications and progress to date of the POWER system is given.

    A primary issue in organizing a new parametric cost analysis function is to determine the skill mix and number of personnel required. The skill mix can be obtained by a functional decomposition of the tasks required within the organization and a matrixed correlation with educational or experience backgrounds. The number of personnel is a function of the skills required to cover all tasks, personnel skill background and cross training, the intensity of the workload for each task, migration through various tasks by personnel along a career path, personnel hiring limitations imposed by management and the applicant marketplace, personnel training limitations imposed by management and personnel capability, and the rate at which personnel leave the organization for whatever reason. Faced with the task of relating all of these organizational facets in order to grow a parametric cost analysis (PCA) organization from scratch, it was decided that a dynamic model was required in order to account for the obvious dynamics of the forming organization. The challenge was to create such a simple model which would be credible during all phases of organizational development. The model development process was broken down into the activities of determining the tasks required for PCA, determining the skills required for each PCA task, determining the skills available in the applicant marketplace, determining the structure of the dynamic model, implementing the dynamic model, and testing the dynamic model.

    Personnel selection involves exchanges of information between job market actors (applicants and organizations). These actors do not have an incentive to exchange accurate information about their ability and commitment to the employment relationship unless it is to their advantage. This state of affairs explains numerous phenomena in personnel selection (e.g., faking). Signaling theory describes a mechanism by which parties with partly conflicting interests (and thus an incentive for deception) can nevertheless exchange accurate information. We apply signaling theory to personnel selection, distinguishing between adaptive relationships between applicants and organizations, among applicants, and among organizations. In each case, repeated adaptations and counteradaptations between actors can lead to situations of equilibrium or escalation (arms races). We show that viewing personnel selection as a network of adaptive relationships among job market actors enables an understanding of both classic and underexplored micro- and macro-level selection phenomena and their dynamic interactions.     The NASA Personnel Security Processing Expert System is a tool that automatically determines the appropriate personnel background investigation required for a civil servant or contractor occupying a position of national security or public trust. It also instructs the personnel security processing staff to perform special checks based on a specific position. The system is implemented using a rule-based expert system and a World Wide Web interface. The system design separates the user interface, knowledge base and control structure to simplify system evolution. When one subsystem is modified, the others are impacted minimally. This system provides many benefits to the NASA Personnel Security Program. First, it frees the agency personnel security specialist from trouble-shooting and correcting all investigative problems. It also provides a learning tool for security processing staff at each installation. The system ensures that each installation security office is in compliance with all applicable laws, regulations and policies. Finally, eliminating overlapping, inappropriate and duplicative efforts to process employees saves many resources. The system was deployed less than a year ago. To date, it saved $1.2 million of the $1.5 million agency-wide personnel security budget.

    The Japan Maritime Self-Defense Force (JMSDF) requires all personnel assigned to Iwo Jima to undergo a preassignment medical examination. The efficacy and benefit of this mandatory examination has not been evaluated objectively in depth. Our purpose was to review the health status of JMSDF personnel assigned to Iwo Jima and to evaluate the correlation of the preassignment medical examination to their current health status for the study period. We divided the participants into two groups. Group Y was composed of JMSDF personnel receiving a preassignment medical examination, and group N was composed of those personnel who did not receive the examination. After the annual health examination, health status was evaluated using objective criteria. We found little statistically significant difference among group Y and group N participants. However, proportionally more patients currently receiving care for or diagnosed with hypertension, hyperuricemia, or severe obesity were identified as being members of group N than group Y. We have demonstrated that the preassignment medical examination may contribute to predicting the health status of potential Iwo Jima personnel and may contribute to controlling the cost of care associated with these specific diagnoses by limiting the assignment of at-risk personnel.     A series of measurements were conducted to determine the cause of a sudden increase in personnel radiation exposures. One objective of the measurements was to determine if the increases were related to changing from film dosimeters exchanged monthly to TLD-100 dosimeters exchanged quarterly. While small increases were observed in the dose equivalents of most employees, the dose equivalents of personnel operating medical electron linear accelerators with energies greater than 20 MV doubled coincidentally with the change in the personnel dosimeter program. The measurements indicated a small thermal neutron radiation component around the accelerators operated by these personnel. This component caused the doses measured with the TLD-100 dosimeters to be overstated. Therefore, the increase in these personnel dose equivalents was not due to changes in work habits or radiation environments. Either film or TLD-700 dosimeters would be suitable for personnel monitoring around high-energy linear accelerators. The final choice would depend on economics and personal preference. PMID:3086255

Quand le dépistage pédiatrique a retenu l'attention, cela a été pour éclairer seulement sur les performances diagnostiques en ignorant même que le test pédiatrique comme bien d'autres peut s'accepter ou se refuser. Cet article met au cœur de son analyse les raisons qui peuvent expliquer qu'on accepte ou qu'on refuse de faire dépister son enfant. Objectif: Etudier chez les parents, les mères, les facteurs explicatifs de l'acceptabilité du test VIH des nourrissons de moins de six mois. Méthodes: Entretien semi-directif à passages répétés avec les parents de nourrissons de moins de six mois dans les formations sanitaires pour la pesée/vaccination et les consultations pédiatriques avec proposition systématique d'un test VIH pour leur nourrisson. Résultats: Nous retenons que la réalisation effective du test pédiatrique du VIH chez le nourrisson repose sur trois éléments. Primo, le personnel de santé par son discours (qui dénote de ses connaissances et perceptions même sur l'infection) orienté vers les mères influence leur acceptation ou non du test. Secundo, la mère qui par ses connaissances et perceptions même sur le VIH, dont le statut particulier, l'impression de bien-être chez elle et son enfant influence toute réalisation du test pédiatrique VIH. Tertio, l'environnement conjugal de la mère, particulièrement caractérisé par les rapports au sein du couple, sur la facilité de parler du test VIH et sa réalisation chez les deux parents ou chez la mère seulement sont autant de facteurs qui influencent la réalisation effective du dépistage du VIH chez l'enfant.     A leading candidate in the detection of special nuclear material (SNM) is active interrogation (AI). Unlike passive interrogation, AI uses a source to enhance or create a detectable signal from SNM (usually fission), particularly in shielded scenarios or scenarios where the SNM has a low activity. The use of AI thus makes the detection of SNM easier or, in some scenarios, even enables previously impossible detection. During the development of AI sources, significant effort is put into determining the source strength required to detect SNM in specific scenarios. Usually during this process, but not always, an evaluation of personnel dose is also completed. In this instance personnel dose could involve any of the following: (1) personnel performing the AI; (2) unknown stowaways who are inside the object being interrogated; or (3) in clandestine interrogations, personnel who are known to be inside the object being interrogated but are unaware of the interrogation. In most instances, dose to anyone found smuggling SNM will be a secondary issue. However, for the organizations performing the AI, legal if not moral considerations should make dose to the personnel performing the AI, unknown stowaways, or innocent bystanders in clandestine interrogations a serious concern.

    The Career History Archival Medical and Personnel System is a database that provides information on cancer, chronic diseases, occupational and preventive medicine, epidemiological research, and the use of health care in the Navy and Marine Corps. It was created at the Naval Health Research Center for enlisted Navy personnel, and it is being expanded to encompass all military personnel. Its objective is to provide a comprehensive, chronologically ordered database of career and medical events in all active duty military service members and to track career and disease events in order from the date of entry to service to the date service ended. Events include the dates of beginning and ending of each specific military occupation, all assignments to a military units or ships, all hospitalized diseases, and other events. The database contains detailed epidemiological data on more than six million members of the military services. It is the largest known epidemiological database in the United States. PMID:15782842

  7. Systematic approach to personnel neutron monitoring

    NTA film and albedo detectors represent the major portion of personnel dosimeters now used for occupational neutron monitoring. However, recent attention to the spectral response of these systems has demonstrated the need for detectors that have a better match to the fields being monitored. Recent developments in direct recoil track etch dosimeters present some intriguing alternatives, and careful use of /sup 237/Np fission fragment detectors offers the advantage of a good dose equivalent spectral match. Work continues on a number of other new detector mechanisms, but problems with sensitivity, energy response, gamma interference, etc., continue to prevent development of most mechanisms into viable personnel dosimeters. Current dosimeter limitations make a systematic approach to personnel neutron monitoring particularly important. Techniques have been developed and tested, using available portable survey instruments, that significantly improve the quality of dosimeter interpretation. Even simple spectrometry can be done with modest effort, significantly improving the health physicists ability to provide accurate neutron monitoring.

    Increased suicide rates for military personnel suffering from post-traumatic stress disorders have been reported in various countries. Although it is known that some peacekeepers are exposed to potentially traumatic events and are thus at risk of suffering from post-traumatic stress reactions, only a few studies have examined suicide rates in this group. Therefore, the aim of this study was to investigate the suicide rate among former Swedish peacekeeping personnel. We compared 39,768 former Swedish peacekeepers to the general population in the National General Population Registry and the Cause-of-Death Registry. A lower number of suicides was found among former Swedish peacekeepers than in the general population. In conclusion, Swedish personnel serving in international peace-keeping operations do not show a higher suicide rate than the general population.
