    The release reporting requirements set out in the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) and the Emergency Planning and Community Right-to-Know Act (EPCRA) enable federal, state, and local authorities to effectively prepare for and respond to chemical accidents. This module reviews the regulations found at 40 CFR Part 302 promulgated pursuant to CERCLA section 103, and the regulations found at 40 CFR section 355.40 promulgated pursuant to EPCRA section 304. The goal of this module is to explain the notification requirements triggered by releases of CERCLA hazardous substances and EPCRA-designated extremely hazardous substances (EHSs).

    President Clinton's Aug. 3, 1993, executive order requires federal agencies to disclose toxic chemical use and release data under Section 313 of the Emergency Planning and Community Right-to-Know Act (EPCRA) and to develop pollution prevention strategies to achieve targeted chemical reduction goals. Section 313 of EPCRA requires manufacturers in Standard Industrial Classification (SIC) codes 20--39 with more than 10 employees to report annually by July 1, 1994, release of listed toxic substances to all environmental media during the previous year. GOCOs that meet these requirements already report Toxics Release Inventory (TRI) data. But non-manufacturing GOCOs and federal facilities not included in SIC codes 20--39 had been exempt from the reporting requirement. The August 3 order does not come as a complete surprise. Last April, President Clinton made several revealing announcements during his Earth Day Address. Perhaps his most salient announcement was his decision to issue an executive order requiring the federal government to become the leader in applying pollution prevention to its daily operations, purchasing decisions and policies, and in reducing toxic releases.

    The University of Hawai‘i (UH) has been collaborating with Okinawa Prefectural Chubu Hospital for over 46 years. This collaboration started as a post-World War II effort to increase the physician workforce. At the initiation of the US Army and State Department, the University of Hawai‘i was recruited, in cooperation with the government of the Ryukyus and USCAR, to initiate a US style postgraduate clinical training program. The Postgraduate Medical Training Program of University of Hawai‘i at Okinawa Chubu Hospital introduced a style of training similar to that in the US by offering a rotating internship. The initial contract had UH establish and run the Postgraduate Medical Training Program of University of Hawaii at Okinawa Central Hospital. After Okinawa's reversion to Japan, under a new contract, UH physicians participated as consultants by providing lectures at “grand rounds” and guidance to faculty, staff, and students. To date, 895 physicians have completed the University of Hawai‘i Postgraduate Medical Training Program with 74 currently training. Approximately 662 (74%) of the trainees have remained in Okinawa Prefecture to practice medicine. As a result, the program has enhanced the physician workforce for the islands of Okinawa and neighbor archipelagos of Miyako and Yaeyama Islands. PMID:24959393

    The Japanese Medical Education system has been influenced by political events throughout the country's history. From long periods of isolation from the western world to the effect of world wars, Japan's training system for physicians has had to adapt in many ways and will continue to change. The Japanese medical education system was recently compared to the “Galapagos Islands” for its unusual and singular evolution, in a speech by visiting professor Dr. Gordon L. Noel at the University of Tokyo International Research center.1 Japanese medical schools are currently working to increase their students' clinical hours or else these students may not be able to train in the United States for residencies. Knowing the history of the Japanese Medical education system is paramount to understanding the current system in place today. Studying the historical foundation of this system will also provide insight on how the system must change in order to produce better clinicians. This article provides a glimpse into the medical system of another nation that may encourage needed reflection on the state of current healthcare training in the United States. PMID:25821652

  12. River networks as biodiversity hotlines.

    For several years, measures to insure healthy river functions and to protect biodiversity have focused on management at the scale of drainage basins. Indeed, rivers bear witness to the health of their drainage basins, which justifies integrated basin management. However, this vision should not mask two other aspects of the protection of aquatic and riparian biodiversity as well as services provided by rivers. First, although largely depending on the ecological properties of the surrounding terrestrial environment, rivers are ecological systems by themselves, characterized by their linearity: they are organized in connected networks, complex and ever changing, open to the sea. Second, the structure and functions of river networks respond to manipulations of their hydrology, and are particularly vulnerable to climatic variations. Whatever the scale considered, river networks represent "hotlines" for sharing water between ecological and societal systems, as well as for preserving both systems in the face of global change. River hotlines are characterized by spatial as well as temporal legacies: every human impact to a river network may be transmitted far downstream from its point of origin, and may produce effects only after a more or less prolonged latency period. Here, I review some of the current issues of river ecology in light of the linear character of river networks. PMID:21640951

    October 2011 marked the 25th Anniversary of the Emergency Planning and Community Right-to-Know Act (EPCRA), which was celebrated for its "significant role in protecting human health and the environment over the last quarter century by providing communities and emergency planners with valuable information on toxic chemical releases in their area." This Note aims to evaluate the effectiveness of three important provisions of the statute-the Toxics Release Inventory, the emergency planning mandate, and the citizen suit provision-through a case study of their implementation in Institute, West Virginia, the site of an industrial accident that prompted the enactment of EPCRA in 1986. This Note argues that although EPCRA made significant improvements to industry transparency in terms of its production and release of hazardous substances, there remain significant barriers concerning adequate resources, informational tools, and enforcement measures. These challenges must be addressed to ensure that citizens are provided with equitable opportunities to inform and ultimately protect their communities from health and environmental hazards. Through interviews with Institute residents and members of a local community advocacy group, along with analyses of the current informational tools available to the public under the statute, the Note will discuss specific challenges facing industrial communities, and offer a series of practical and legal solutions to increase the effectiveness of the statute, particularly in the most economically and politically vulnerable communities.     This guidance document updates and clarifies information provided in an earlier guidance document published by the US Environmental Protection Agency (EPA) entitled Guidance for Federal Facilities on Release Notification Requirements under CERCLA and SARA Title III (EPA 9360.7-06; November 1990). Since publication of that earlier guidance document, several significant events have occurred that affect the reporting obligations of facilities owned or operated by the Department of Energy (DOE), including the publication of Executive Order 12856--Federal Compliance with Right-to-Know Laws and Pollution Prevention Requirements--and a rejection by the US Court of Appeals of EPA`s interpretation of the term release into the environment. In preparing this guidance document, the Office of Environmental Policy and Assistance, RCRA/CERCLA Division (EH-413), has documented responses to queries from DOE field elements on CERCLA and EPCRA release reporting requirements, as well as incorporating those Questions and Answers from the previous document that remain germane to DOE`s reporting obligations under CERCLA and EPCRA.

  18. Implementation and Early Utilization of a Suicide Hotline for Veterans

    Suicide crisis lines have a respected history as a strategy for reducing deaths from suicide and suicidal behaviors. Until recently, however, evidence of the effectiveness of these crisis lines has been sparse. Studies published during the past decade suggest that crisis lines offer an alternative to populations who may not be willing to engage in treatment through traditional mental health settings. Given this promising evidence, in 2007, the Department of Veterans Affairs in collaboration with the Department of Health and Human Services’ Substance Abuse and Mental Health Administration implemented a National Suicide Hotline that is staffed 24 hours a day, 7 days a week, by Veterans Affairs clinical staff. We report here on the implementation of this suicide hotline and our early observations of its utilization in a largely male population. PMID:22390596

    Public relations activities of the German Research Network on Schizophrenia (GRNS) have shown that there is a demand for more information about schizophrenia disorder. This confirms international research findings that relatives of schizophrenia patients are particularly in need of information and support. In response, the GRNS has maintained a telephone hotline since 2001. The hotline is manned by clinical experts, psychiatrists, or psychologists once a week. The telephone calls are documented in a systematic manner. From 2001 to 2003, 3,909 calls were registered. This volume exceeds the limit of the hotline's resources. The telephone hotline is mainly used by relatives of psychotic patients. Most questions relate to the symptoms of schizophrenia and pharmaceutical treatment. The need for emotional support is also a high motivational factor for dialing the hotline number. The telephone hotline seems to be a worthwhile addition to the already existing crisis telephones and should be maintained even after public funding of the network expires. PMID:16028078

    The first 24-hour AIDS hotline in the Arab world will open in Cairo, Egypt, in October 1995. The opening of the new service marks a change in attitude on the part of the Egyptian government, which has maintained a discreet AIDS control program in the past. Approval from religious leaders was necessary for the new program to begin; the United Nations Children's Fund (UNICEF) played a prominent role in negotiations. The "Counsel and Hot Line Centre," which will be based in Imbala district, will employ 19 people, including two doctors and two psychologists. The Centre was funded with US$300,000 from the Ford Foundation. Currently, 478 persons with HIV infections and 110 people with AIDS have been reported. The ministry estimates that there are 5000-7000 persons with HIV infections in Egypt. Although these figures were greeted with suspicion by organizations outside of Egypt, subsequent testing has indicated low prevalence rates for this country, despite high tourism and a large population of migrant workers.   11. Flow rates and warming efficacy with Hotline and Ranger blood/fluid warmers.

    The heating capabilities of a water bath blood/fluid warmer, Hotline, have proven superior to those of other devices. The dry heat warmer Ranger has not previously been compared with the Hotline. We evaluated these devices in terms of flow rates and efficacy of warming. We delivered room temperature (21 degrees C) saline and 10 degrees C packed red blood cells (RBCs) by using 90 mm Hg (gravity equivalent) and 300 mm Hg bag pressure and various sizes of IV catheters. The outflow from each device was connected to an inline thermistor, and simultaneous measurements of outflow temperature and flow volume per minute were recorded. Additional data points were obtained with a roller pump that delivered flows of 1-6 L/h through each device. We calculated the effect of these flow rates and outflow temperatures on the mean body temperature (MBT) of a 70-kg patient. The Hotline and Ranger had similar flow rates at 90 and 300 mm Hg pressure infusion when studied with various sizes of IV catheters. Hotline was able to deliver warmer RBCs and saline at slower flow rates (1-4 L/h), but because changes in MBT were almost identical, there was no clinically important advantage, and almost no heat was transferred at these slower flow rates. At more rapid flow rates (>4 L/h), the Ranger warmed RBCs and saline better and produced smaller decreases in MBT than the Hotline. The use of the Hotline for rapid infusions, especially of cold RBCs, is not recommended because of low outflow temperatures and decreases in MBT that were three times larger than those seen with the Ranger.     The Homeland Security Advisory System (HSAS) was established by the Department of Homeland Security to communicate the risk of a terrorist event. In order to explore the potential psychological impacts of HSAS we analyzed the effects of terror alerts on the law enforcement community. We used data from the New Jersey Cop 2 Cop crisis intervention hotline. Incidence Rate Ratios--interpreted as average relative increases in the daily number of calls to the Cop 2 Cop hotline during an increased alert period--were computed from Poisson models. The hotline received a total of 4,145 initial calls during the study period. The mean daily number of calls was higher during alert level elevation compared to prior 7 days (7.68 vs. 8.00). In the Poisson regression analysis, the Incidence Rate Ratios of number of calls received during elevated alert levels compared to the reference period of seven days preceding each change in alert were close to 1, with confidence intervals crossing 1 (i.e. not statistically significant) for all lag periods evaluated. This investigation, in the context of New Jersey law enforcement personnel, does not support the concern that elevating the alert status places undue stress upon alert recipients. PMID:18459529

  2. Safe abortion information hotlines: An effective strategy for increasing women's access to safe abortions in Latin America.

    This paper describes the implementation of five Safe Abortion Information Hotlines (SAIH), a strategy developed by feminist collectives in a growing number of countries where abortion is legally restricted and unsafe. These hotlines have a range of goals and take different forms, but they all offer information by telephone to women about how to terminate a pregnancy using misoprostol. The paper is based on a qualitative study carried out in 2012-2014 of the structure, goals and experiences of hotlines in five Latin American countries: Argentina, Chile, Ecuador, Peru and Venezuela. The methodology included participatory observation of activities of the SAIH, and in-depth interviews with feminist activists who offer these services and with 14 women who used information provided by these hotlines to induce their own abortions. The findings are also based on a review of materials obtained from the five hotline collectives involved: documents and reports, social media posts, and details of public demonstrations and statements. These hotlines have had a positive impact on access to safe abortions for women whom they help. Providing these services requires knowledge and information skills, but little infrastructure. They have the potential to reduce the risk to women's health and lives of unsafe abortion, and should be promoted as part of public health policy, not only in Latin America but also other countries.   3. Implications of National Anesthesia Workload on the Staffing of a Call Center: The Malignant Hyperthermia Consultant Hotline.

    Recently, we analyzed data from the American Society of Anesthesiologist's (ASA) Anesthesia Quality Institute (AQI) to report the United States (U.S.) anesthesia workload by time of day and day of the week. The AQI data were reported using the Central Time zone. Times for the N = 613 calls to the Malignant Hyperthermia Association of the United States (MHAUS) Malignant Hyperthermia (MH) Hotline from August 1, 2012, through March 7, 2014, were adjusted similarly. The MH Hotline effectively provides at all times to each anesthesia group an additional board-certified anesthesiologist who has expertise in managing, diagnosing, and/or preventing MH crises. We compared the timing of calls with the MH Hotline consultants relative to times of most anesthesia workload nationally. The interval 6:30 AM to 6:30 PM Central Time on regular workdays accounted for most (P < 0.0001) calls to the MH Hotline (62.5% ± 2.0% [mean ± standard error]). However, the interval accounted for significantly less than the 82.2% of anesthesia minutes and 84.5% of general anesthesia minutes during that interval nationally (both P < 0.0001). Thus, most calls to the MH Hotline occurred when anesthesia groups nationwide were the busiest. Weekends accounted for 15.3% ± 1.5% of MH Hotline calls, significantly greater than the rates of 5.2% of anesthesia minutes and 4.3% of general anesthesia minutes during weekends nationally (both P < 0.0001). Thus, the MH Hotline was used proportionately more often when anesthesia providers have fewer colleagues present and available for consultation (all P < 0.0001).   8. A quality improvement project to tackle under-reporting of hazards by doctors by using an anonymous telephone hotline

    An anonymous hazard reporting hotline was established for doctors to use on two general medical wards at Weston General Hospital (WGH) in North Somerset in England during an eight day period in July/August 2014. Doctors were encouraged to report hazards and near misses or just start a conversation about a concern. The existing computer based significant event system remained alongside the hotline and was to be used where actual harm had come to a patient. A team of consultants and quality improvement administrators monitored the hotline on a daily basis, categorized the level of risk being reported and ensured, where possible, that action was taken to address the doctors' concerns. The hotline increased reporting rates from two per month to 25 in eight days. The system identified 13 amber (medium risk) and five red (high risk) hazards which would otherwise not have been known about. This author believes these five high risk reports alone make the case for a new, quicker, easier, and anonymous reporting system for doctors at WGH urgent and irrefutable. PMID:26893901

  9. Description of calls from private well owners to a national well water hotline, 2013.

    Water Systems Council (WSC) is a national, non-profit organization providing education and resources to private household well owners. Since 2003, WSC has provided wellcare®, a toll-free telephone hotline to answer questions from the public regarding well stewardship. In order to identify knowledge gaps regarding well stewardship among private well owners, we obtained data from WSC and reviewed calls made during 2013 to wellcare®. WSC records data from each wellcare® call-including caller information, primary reason for call, main use of well water, and if they were calling about a cistern, private well, shared well, or spring. We searched for calls with key words indicating specific contaminants of interest and reviewed primary reasons for calls. Calls classified as primarily testing-related were further categorized depending on whether the caller asked about how to test well water or how to interpret testing results. During 2013, wellcare® received 1100 calls from private well owners who were residents of 48 states. Among these calls, 87 (8%) mentioned radon, 83 (8%) coliforms, 51 (5%) chemicals related to fracking, 34 (3%) arsenic, and 32 (3%) nitrates key words. Only 38% of private well owners reported conducting any well maintenance activities, such as inspecting, cleaning, repairing the well, or testing well water, during the previous 12 months. The primary reason for calls were related to well water testing (n=403), general information relating to wells (n=249), contaminants (n=229), and well water treatment (n=97). Among calls related to testing, 319 had questions about how to test their well water, and 33 had questions about how to interpret testing results. Calls from private well owners to the wellcare® Hotline during 2013 identified key knowledge gaps regarding well stewardship; well owners are generally not testing or maintaining their wells, have questions about well water testing treatment, and concerns about well water contaminants.   10. Patterns and predictors of smoking cessation among users of a telephone hotline.

    Most former cigarette smokers in the United States have stopped without formal assistance. However, a large proportion of smokers desire and seek help other than by attending formal programs. It is important to recognize what factors are likely to influence the effectiveness of smoking cessation attempts among these persons. The authors report results of a prospective cohort study of 1,552 smokers who called a stop smoking hotline to request self-help smoking cessation information. The participants were classified into three groups based on reports at the 6-month followup: 242 quitters, 497 recidivists, and 813 nonquitters. Baseline and followup data were used to evaluate three comparisons: quitters versus nonquitters, quitters versus recidivists, and recidivists versus nonquitters. Nonquitters appear to be less motivated and more doubtful of their abilities to quit successfully compared with the other two groups. Quitters appear to live in a supportive environment for smoking cessation. Heavier smokers are more hesitant to try to quit, but once they make an attempt they are as likely to succeed as lighter smokers, when other factors are kept constant. Efforts to promote environments supportive of smoking cessation are likely to result in a larger number of successful quitters. Similarly, efforts to strengthen motivation and belief in personal ability to quit are likely to encourage more nonquitters to attempt to stop smoking. Finally, it appears that some smokers need a previous quit attempt before they are able to maintain cessation successfully. PMID:8265763

  11. WEAMR — A Weighted Energy Aware Multipath Reliable Routing Mechanism for Hotline-Based WSNs

    Reliable source to sink communication is the most important factor for an efficient routing protocol especially in domains of military, healthcare and disaster recovery applications. We present weighted energy aware multipath reliable routing (WEAMR), a novel energy aware multipath routing protocol which utilizes hotline-assisted routing to meet such requirements for mission critical applications. The protocol reduces the number of average hops from source to destination and provides unmatched reliability as compared to well known reactive ad hoc protocols i.e., AODV and AOMDV. Our protocol makes efficient use of network paths based on weighted cost calculation and intelligently selects the best possible paths for data transmissions. The path cost calculation considers end to end number of hops, latency and minimum energy node value in the path. In case of path failure path recalculation is done efficiently with minimum latency and control packets overhead. Our evaluation shows that our proposal provides better end-to-end delivery with less routing overhead and higher packet delivery success ratio compared to AODV and AOMDV. The use of multipath also increases overall life time of WSN network using optimum energy available paths between sender and receiver in WDNs. PMID:23669714

    This study presents preliminary evidence that exposure to different health campaigns enhance intentions to seek help from telephone hotlines among Veteran households. Data were collected from telephone surveys (N = 8,756) conducted with both Veteran (n = 3,904), and for comparison, non-Veteran households (n = 4,852). Cox proportional hazard models were used to identify associations between message exposure variables ("type" or "number") with a high intent to use different hotlines (e.g., suicide prevention, domestic violence). As the number of types of messages an individual was exposed to increased, reported high intent for hotline use also increased. This remained significant across hotline type and for both Veteran and non-Veterans households. Results underscore the need for further research on dissemination strategies of public messaging and their impact on health behavior among Veteran populations. PMID:27391618

    Background Telephone hotlines designed to address common concerns in the early postpartum could be a useful resource for parents. Our aim was to test the feasibility of using a telephone as an intervention in a randomized controlled trial. We also aimed to test to use of algorithms to address parental concerns through a telephone hotline. Methods Healthy first-time mothers were recruited from postpartum wards of hospitals throughout Lebanon. Participants were given the number of a 24-hour telephone hotline that they could access for the first four months after delivery. Calls were answered by a midwife using algorithms developed by the study team whenever possible. Callers with medical complaints were referred to their physicians. Call patterns and content were recorded and analyzed. Results Eighty-four of the 353 women enrolled (24%) used the hotline. Sixty percent of the women who used the service called more than once, and all callers reported they were satisfied with the service. The midwife received an average of three calls per day and most calls occurred during the first four weeks postpartum. Our algorithms were used to answer questions in 62.8% of calls and 18.6% of calls required referral to a physician. Of the questions related to mothers, 66% were about breastfeeding. Sixty percent of questions related to the infant were about routine care and 23% were about excessive crying. Conclusions Utilization of a telephone hotline service for postpartum support is highest in the first four weeks postpartum. Most questions are related to breastfeeding, routine newborn care, and management of a fussy infant. It is feasible to test a telephone hotline as an intervention in a randomized controlled trial. Algorithms can be developed to provide standardized answers to the most common questions. PMID:20946690

    This paper deals with a power supply using switching modulation technique, and its application to the hot-line insulation diagnosis. One of the distinct features of the switching modulation techinque employed in this study is that the zero-phase-sequence voltage with arbitary frequency is generated easily in the three-phase power system. Basic composition is just to add a switching modulation device to the commercial power supply or charged indoor main line. Therefore, it is not necessary to feed a signal current from the signal power supply to the grounding conductor via a pouring transformer as required by current hot-line insulation diagnosis system. When a power supply using switching modulation technique is used to hot-line insulation diagnosis, it is not necessary to modify or change existing commercial power facility. Therefore, there is a possibility that this may advantageously be used as a power supply for mobile type diagnosis. This paper deals with basic theory of switching modulation technique, and the phase rotation characteristics which play a basic role in three-phase power supply, and summarizes basic composition and performances of a trial manufactured experimental device. Furthermore, results of discussion are introduced concerning the conditions and method of its apllication to indoor wiring of fatories and buildings.

    appear to be the main construction material of each chain; anomalously high modern heatflow occurs along their lengths; and hotspot-like age progression of volcanism is not clearly defined along their lengths. It is apparent that the CVL is not the product of a single mantle plume or hotspot, and we speculate that the CVL and possibly other NE-trending volcanic chains off West Africa (and perhaps linear belts of Neogene volcanism on the African continent) are the result of linear, mantle upwelling zones or 'hotlines'. These hotlines are suggested to form above upwelling flow currents in between cylindrical Rayleigh-Bernard convection rolls in the upper mantle. Such convection may be driven by heat transfer across and/or shear along the 670 km discontinuity as a result of convection in the lower mantle.

    The rates of melanomas and skin cancers are increasing in the United States. Children attending elementary schools are in the most danger of acquiring these diseases later in life, and elementary school children in Hawai‘i have the greatest risk of all children in the United States. The parents and educators of Hawai‘i's elementary school age children are unaware of the potential risks for cancer that young children experience every day at school. Effective sun protection policies have been implemented in other jurisdictions, including Australia, that have similar risks for over-exposure to solar ultraviolet radiation in children. These proven policy models can inform sun protection practices in Hawai‘i. A simple policy whereby public elementary schools require that children wear ordinary long sleeves shirts and hats during the school's outdoor activities will protect Hawai‘i's children from overexposure to sun's ultraviolet radiation. Establishment of a state law codifying the implementation of this simple, yet scientifically proven strategy into the policies of Hawai‘i's public elementary schools can significantly reduce the incidence and deaths from melanoma and skin cancer in the state.     We examined the impact of the implementation of Applied Suicide Intervention Skills Training (ASIST) across the National Suicide Prevention Lifeline's national network of crisis hotlines. Data was derived from 1,507 monitored calls from 1,410 suicidal individuals to 17 Lifeline centers in 2008–2009. Callers were significantly more likely to feel less depressed, less suicidal, less overwhelmed, and more hopeful by the end of calls handled by ASIST-trained counselors. Few significant changes in ASIST-trained counselors’ interventions emerged; however, improvements in callers’ outcomes were linked to ASIST-related counselor interventions, including exploring reasons for living and informal support contacts. They give out a voltage impulse when struck, the greater the force of impact, the higher the voltage. The impulse is transmitted to a compact electronics package where voltage is translated into a force-pounds reading shown on a digital display.     Training is a major endeavor in all modern societies. Common training methods include training manuals, formal classes, procedural computer programs, simulations, and on-the-job training. NASA's training approach has focussed primarily on on-the-job training in a simulation environment for both crew and ground based personnel. NASA must explore new approaches to training for the 1990's and beyond. Specific autonomous training systems are described which are based on artificial intelligence technology for use by NASA astronauts, flight controllers, and ground based support personnel that show an alternative to current training systems. In addition to these specific systems, the evolution of a general architecture for autonomous intelligent training systems that integrates many of the features of traditional training programs with artificial intelligence techniques is presented. These Intelligent Computer Aided Training (ICAT) systems would provide much of the same experience that could be gained from the best on-the-job training.

    Cognitive training studies yield wildly inconsistent results. One dimension on which studies vary is the scheduling of training sessions (Morrison and Chein, 2011). In this study, we systematically address whether or not spacing of practice influences training and transfer. We randomly assigned 115 fifth grade children to an active control group or one of four training groups who received working memory training based on a “running span” task (Zhao et al., 2011). All groups received the same total amount of training: 20 sessions of training with 60 trials for an average of 20 min per session. The training was spread across 2, 5, 10, or 20 days. The active control group received 20-min sessions of math instruction for 20 sessions. Before and after training participants in all five groups performed a single transfer test that assessed fluid intelligence, the Raven's Progressive Matrices Test. Overall, participants in all four training groups improved significantly on the training task (at least partially), as reflected by increased speed. More importantly, the only training group to show significant improvement on the Raven's was the group who had the greatest amount of spacing (20 days group) during training and improvement in this group was significantly higher than that of the control group.     Training is a dynamic process to assure that personnel are capable of performing their assigned functions. CGMP regulations contain only general expectations, and no FDA guideline regarding training has been issued. Training programs are generally in place in pharmaceutical firms. However, training quality and effectiveness may be inadequate in a number of firms. A pharmaceutical firm should be able to show that its training program consistently meets its training goals as purported, and that each trainee completing an instructional module has acquired the competencies as purported. The proper application of sound principles of instructional design should help firms overcome GMP deficiencies regarding training and personnel qualification. For example, principles of mastery learning, competency-based instruction, performance objectives, a systems approach to instructional design, and the evaluation of instruction as well as the instructional program should help ensure meaningful, relevant training and appropriate, effective instruction. Review of training should be included in the firm's program for managing change. Firms should also ensure adequate training documentation, a positive attitude toward training, and that training is not used inappropriately to remedy performance deviations not resulting from skills deficiencies. As crews become more autonomous, their potential span of control and required expertise must grow to match their autonomy. It is not possible to train for every eventuality ahead of time on the ground, or to maintain trained skills across long intervals of disuse. To adequately prepare NASA personnel for these challenges, new training approaches, methodologies, and tools are required. This research project aims at developing these training capabilities. By researching established training principles, examining future needs, and by using current practices in space flight training as test beds, both in Flight Controller and Crew Medical domains, this research project is mitigating program risks and generating templates and requirements to meet future training needs. Training efforts in Fiscal Year 09 (FY09) strongly focused on crew medical training, but also began exploring how Space Flight Resource Management training for Mission Operations Directorate (MOD) Flight Controllers could be integrated with systems training for optimal Mission Control Center (MCC) operations.     The U.S. government has been assisting the Russian Federation (RF) Ministry of Defense (MOD) for many years with nuclear weapons transportation security (NWTS) through the provision of specialized guard escort railcars and cargo railcars with integrated physical security and communication systems, armored transport vehicles, and armored escort vehicles. As a natural continuation of the NWTS program, a partnership has been formed to construct a training center that will provide counterterrorism training to personnel in all branches of the RF MOD. The Abramovo Counterterrorism Training Center (ACTC) is a multinational, multiagency project with funding from Canada, RF and the U.S. Departments of Defense and Energy. ACTC will be a facility where MOD personnel can conduct basic through advanced training in various security measures to protect Category IA material against the threat of terrorist attack. The training will enhance defense-in-depth principles by integrating MOD guard force personnel into the overall physical protection systems and improving their overall response time and neutralization capabilities. The ACTC project includes infrastructure improvements, renovation of existing buildings, construction of new buildings, construction of new training facilities, and provision of training and other equipment. Classroom training will be conducted in a renovated training building. Basic and intermediate training will be conducted on three different security training areas where various obstacles and static training devices will be constructed. The central element of ACTC, where advanced training will be held, is the 'autodrome,' a 3 km road along which various terrorist events can be staged to challenge MOD personnel in realistic and dynamic nuclear weapons transportation scenarios. This paper will address the ACTC project elements and the vision for training development and integrating this training into actual nuclear weapons transportation operations.

    For a speech therapist, the breathing method is one of the important techniques to train a person who has difficulty in speaking. Insufficient number of therapists makes it difficult to apply these breath training methods widely. Therefore, development of a new breath training method will be mandatory. With this reason, a new training system is developed and described in this paper. In this new method, expiration pattern of a speech therapist analyzing through an air flow sensor is constructed into visual graphic image in order to recognize expiration pattern easily. With experimental use of this system to without handicapped trainees, they could successfully imitate trainer's pronunciation easily. A person who has difficulty in speaking will be able to learn easily how to do expiration for speaking through visual pattern.

    We analyze the physics of the "world's simplest electric train." The "train" consists of a AA battery with a strong magnet on each end that moves through a helical coil of copper wire. The motion of the train results from the interaction between the magnetic field created by the current in the wire and the magnetic field of the magnets. We calculate the force of this interaction and the terminal velocity of the train due to eddy currents and friction. Our calculations provide a good illustration of Faraday's and Lenz's laws, as well as of the concepts of the Lorentz force and eddy currents.

TMIS is at the leading edge of training information systems used in the nuclear industry. The database contains all the necessary records to confirm the department's adherence to accreditation criteria and houses all test questions, student records and information needed to evaluate the training process. The key to the TMIS system is that the impact of any change (i.e., procedure change, new equipment, safety incident in the commercial nuclear industry, etc.) can be tracked throughout the training process.     Federal public health advisors provide guidance and assistance to health departments to improve public health program work. The Centers for Disease Control and Prevention (CDC) prepares them with specialized training in administering public health programs. This article describes the evolving training and is based on internal CDC documents and interviews. The first federal public health advisors worked in health departments to assist with controlling syphilis after World War II. Over time, more CDC prevention programs hired them. To meet emerging needs, 3 major changes occurred: the Public Health Prevention Service, a fellowship program, in 1999; the Public Health Associate Program in 2007; and integration of those programs. Key components of the updated training are competency-based training, field experience, supervision, recruitment and retention, and stakeholder support.     Federal public health advisors provide guidance and assistance to health departments to improve public health program work. The Centers for Disease Control and Prevention (CDC) prepares them with specialized training in administering public health programs. This article describes the evolving training and is based on internal CDC documents and interviews. The first federal public health advisors worked in health departments to assist with controlling syphilis after World War II. Over time, more CDC prevention programs hired them. To meet emerging needs, 3 major changes occurred: the Public Health Prevention Service, a fellowship program, in 1999; the Public Health Associate Program in 2007; and integration of those programs. Key components of the updated training are competency-based training, field experience, supervision, recruitment and retention, and stakeholder support.     Effective training by simulation is a matter of 1. accurate definition of the training need, 2. selecting the right hardware/software package to do the job cost effectively, 3. careful, methodical building of the training program and 4. an appropriate setting, such as a well-equipped training center. Several points seem clear: 1. ''Computer shock'' or computer phobia -- that is, fear of the simulator, can be a factor with some of the older operators. But it quickly disappears once they're into the program, and learn the versatility and the exciting dynamics of simulators. 2. Operator training is becoming more sophisticated throughout the HPI. Among other things, operators should get out into the plant to learn more about how their actions in the control room impact the plant. Operators are becoming more ''big picture'' oriented. 3. When inexperienced training instructors are used, they should attend a good ''train the trainer'' course emphasizing platform skills and instructional technology. 4. Operators need as much ''hands on'' experience with the plant's actual operating equipment as possible. The cognitive ''linkage'' between that equipment and what they see on the CRT is vitally important.

    The California Department of Health Services' Occupational Health Branch and others have identified the construction industry as being at high risk for injuries, illnesses, and fatalities. Effective tailgate trainings (brief job site safety meetings) can be a powerful tool to promote hazard awareness and safe work practices. The authors found that many contractors and supervisors conducted ineffective tailgate trainings. They developed the BuildSafe California Project to assist contractors to have more effective programs by holding 25 training-of-trainers sessions reaching 1,525 participants. The needs assessment, intervention, and evaluation results from the first 18 trainings are presented. Eighty-six percent of the participants found the program "very helpful." Participants used the materials and made improvements in the quality and frequency of trainings. Supervisors must be skilled at conducting tailgate trainings as part of their responsibilities. There is a serious need to provide more culturally appropriate safety training in a workforce increasingly made up of Latino workers. PMID:18372431

    The prosecution of modern war and the competing missions of peacekeeping, humanitarian missions, and beneficiary care place great demands on the military medical system. Meeting the military medical training challenges of the new millennium requires the best trained and most experienced medical personnel possible. Various strategies for initial and sustainment (continuing) medical training are available to ensure that the medical force is ready for the next mission. Accredited programs both in and out of the military are the mainstay of training for both enlisted personnel and officers, with professional certification serving as the standard for competency. Clinical sustainment training can take place in military medical treatment facilities, civilian institutions, or a combination of the two. When direct patient care opportunities cannot provide the proper mixture of experiences to maintain certain skills, short courses, distance education, and patient simulators can play important roles. Because each training strategy offers certain advantages in different settings, it is likely that military medical departments will need to use all of them. An optimal training environment benefits from all strategies used, in combination or separately. PMID:16435752

    Teledyne Brown developed a computer-based interactive multimedia training system for use with the Crystal Growth Furnace in the U.S. Microgravity Laboratory-2 mission on the Space Shuttle. Teledyne Brown commercialized the system and customized it for PPG Industries Aircraft Products. The system challenges learners with role-playing scenarios and software-driven simulations engaging all the senses using text, video, animation, voice, sounds and music. The transfer of this technology to commercial industrial process training has resulted in significant improvements in effectiveness, standardization, and quality control, as well as cost reductions over the usual classroom and on-the- job training approaches.

By using coaching techniques and strategies gleaned from analysis of professional music ensembles and athletic training, surgical educators can set conditions that increase the success rate of training to elite performance. This article describes techniques and strategies used in both music and athletic coaching, and it discusses how they can be applied and integrated into surgical simulation and education.   15. Meeting training needs through operator training simulators

    An Operator Training Simulator (OTS) can be used to train operators how to safely operate their process plant. Of particular concern are the most critical plant operations and upsets which occur very infrequently during the course of normal every day operations. In that context, the OTS is similar in concept to federally mandated flight simulators which are used to certify commercial airline pilots in take-off, landing and emergency conditions. Of the 170 largest industry losses, 127 occurred during normal operation, while the remaining 43 occurred during other activities. When these figures are combined with the fact that the industry average on-stream factor is 94%, an accident is over five times more likely to occur during other than normal plant operation. The costs associated with atypical plant operation accidents (start-up, shutdown, severe upsets, etc.) are 50% higher than those which occur during normal operation. These statistics combined with new OSHA training requirements suggest that operating companies must now take additional measures to ensure that their operating staff are totally familiar with abnormal process operations, particularly handling of process upsets and emergency operating procedures.

    Line oriented training is used in a broader, more generic sense that as a specific program under FAR 12.1409 and AC 120-35. A company policy was adopted more than twenty years ago requiring that all pilot checks and recurrent training be conducted with a full crew occupying the seats they occupy on the line. Permission was obtained to reschedule the hours for recurrent proficiency training to include one and one-half hours of LOFT flight. The number of emergencies and abnormal procedures which could be undertaken were considered and the introduction of an a occasional incapacitation revealed which person is the most difficult to replace on the widebodies. By using the LOFT concept, every training period can be structured like a typical line flight. The use of LOFT in simulator syllabus development and problems that need to be refined are discussed.

Most EU-memberstates are not a proponent for a vocational year after the basic dental training. Deficiencies in the basic dental training should have been met by lengthening the basic training.
