    Externalities has become the catchword for a major experiment in electric utility regulation. Together with increased competition as a means for economic regulation, this experiment represents a potential revolution in how electric utilities are regulated. It is very important for utilities and policy makers to understand the technical issues and arguments driving the externality experiment. This Technical Review presents four papers covering topics in economics that may play important roles in this revolution. The four papers are: Economic Issues in the Application of Externalities to Electricity Resource Selection; Climate Change, the Marginal Cost of Carbon Dioxide Emissions and the Implications for Carbon Dioxide Emissions Adders; Positive Externalities and Benefits from Electricity; and Socioeconomic Effects of Externality Adders for Electric Utility Emissions.

    The U.S. Department of Energy's Office of Environmental Management (DOE-EM) was established to achieve the safe and compliant disposition of legacy wastes and facilities from defense nuclear applications. A large majority of these wastes and facilities are 'one-of-a-kind' and unique to DOE. Many of the programs to treat these wastes have been 'first-of-a-kind' and unprecedented in scope and complexity. This has meant that many of the technologies needed to successfully disposition these wastes were not yet developed or required significant re-engineering to be adapted for DOE-EM's needs. The DOE-EM program believes strongly in reducing the technical risk of its projects and has initiated several efforts to reduce those risks: (1) Technology Readiness Assessments to reduce the risks of deployment of new technologies; (2) External Technical Reviews as one of several steps to ensure the timely resolution of engineering and technology issues; and (3) Technical Risk Ratings as a means to monitor and communicate information about technical risks. This paper will present examples of how Technology Readiness Assessments, External Technical Reviews, and Technical Risk Ratings are being used by DOE-EM to reduce technical risks.

    An External Technical Review (ETR) team evaluated the system-level modeling and simulation tools in support of Savannah River Site and Office of River Protection liquid waste processing and disposal systems. The reviews focused on the following primary areas: software and modeling tools, capability to model needed facilities and operations, rate at which predictions are performed, and need for additional tools. The Liquid Waste Disposition Systems at these sites are highly integrated operations that involve safely storing liquid waste in underground storage tanks; removing, treating, and disposing low-activity fraction on-site; vitrifying high-level waste; and storing the vitrified waste until permanent disposition at a Federal Repository. The purpose of the ETR team was to evaluate the current process simulation tools that support the planning basis for life cycle liquid waste disposition system plans. These plans establish a base for processing the constituents of liquid waste systems to the end of the program mission. Observations and recommendations were made for each of the review areas. In general, tools appear to provide reasonable estimates for their intended purposes; however, additional functionality and new tools are needed, as well as greater flexibility and improved performance in producing estimates for planning purposes.

    The request to conduct an independent review of regression models, developed for determining the expected Launch Commit Criteria (LCC) External Tank (ET)-04 cycle count for the Space Shuttle ET tanking process, was submitted to the NASA Engineering and Safety Center NESC on September 20, 2005. The NESC team performed an independent review of regression models documented in Prepress Regression Analysis, Tom Clark and Angela Krenn, 10/27/05. This consultation consisted of a peer review by statistical experts of the proposed regression models provided in the Prepress Regression Analysis. This document is the consultation's final report.

This document describes the technical basis for the WIPP External Radiation Dosimetry Program. The purposes of this document are to: (1) provide assurance that the WIPP External Radiation Dosimetry Program is in compliance with all regulatory requirements, (2) provide assurance that the WIPP External Radiation Dosimetry Program is derived from a sound technical base, (3) serve as a technical reference for radiation protection personnel, and (4) aid in identifying and planning for future needs. The external radiation exposure fields are those that are documented in the WIPP Final Safety Analysis Report.

    A review of the Oconee-3 Probabilistic Risk Assessment (OPRA) was conducted with the broad objective of evaluating qualitatively and quantitatively (as much as possible) the OPRA assessment of the important sequences that are ''externally'' generated and lead to core damage. The review included a technical assessment of the assumptions and methods used in the OPRA within its stated objective and with the limited information available. Within this scope, BNL performed a detailed reevaluation of the accident sequences generated by internal floods and earthquakes and a less detailed review (in some cases a scoping review) for the accident sequences generated by fires, tornadoes, external floods, and aircraft impact. 12 refs., 24 figs., 31 tabs.

    The Aircraft Digital Flight Control Technical Review was initiated by two pilot induced oscillation (PIO) incidents in the spring and summer of 1992. Maj. Gen. Franklin (PEO) wondered why the Air Force development process for digital flight control systems was not preventing PIO problems. Consequently, a technical review team was formed to examine the development process and determine why PIO problems continued to occur. The team was also to identify the 'best practices' used in the various programs. The charter of the team was to focus on the PIO problem, assess the current development process, and document the 'best practices.' The team reviewed all major USAF aircraft programs with digital flight controls, specifically, the F-15E, F-16C/D, F-22, F-111, C-17, and B-2. The team interviewed contractor, System Program Office (SPO), and Combined Test Force (CTF) personnel on these programs. The team also went to NAS Patuxent River to interview USN personnel about the F/A-18 program. The team also reviewed experimental USAF and NASA systems with digital flight control systems: X-29, X-31, F-15 STOL and Maneuver Technology Demonstrator (SMTD), and the Variable In-Flight Stability Test Aircraft (VISTA). The team also discussed the problem with other experts in the field including Ralph Smith and personnel from Calspan. The major conclusions and recommendations from the review are presented.

    A novel laser sensor for position measurements of technical solid-state surfaces is proposed. An external Fabry-Perot laser cavity is assembled by use of an antireflection-coated laser diode together with the technical surface. Mode locking results from pumping the laser diode synchronously to the mode spacing of the cavity. The laser cavity length, i.e., the distance to the measurement object, is determined by evaluation of the modulation transfer function of the cavity by means of a phase-locked loop. The mode-locking external-cavity laser sensor incorporates a resonance effect that results in highly resolving position and displacement measurements. More than a factor-of-10 higher resolution than with conventional nonresonant sensing principles is achieved. Results of the displacement measurements of various technical surfaces are reported. Experimental and theoretical investigations are in good agreement.

    This report documents an Independent Technical Review (ITR) of the facilities, organizations, plans, and activities required to transition particular elements of the Mound Plant from Defense Program (DP) funded operation as appropriate either to community developed reuse or safe deactivation leading to decontamination and decommissioning (D&D). The review was conducted at the request of the Dr. Willis Bixby, Deputy Assistant Secretary, U.S. Department of Energy EM-60, Office of Facility Transition and Management and is a consensus of the nine member ITR Team. Information for the review was drawn from documents provided to the ITR Team by the Miamisburg Area Office (MB) of the DOE, EG&G, the City of Miamisburg, and others; and from presentations, discussions, interviews, and facility inspections at the Mound Plant during the weeks of March 14 and March 28, 1994. During the week of April 25, 1994, the ITR Team met at Los Alamos, New Mexico to develop consensus recommendations. A presentation of the core recommendations was made at the Mound Plant on May 5, 1994. This is an independent assessment of information available to, and used by, the Mound Plant personnel. Repetition of the information is not meant to imply discovery by the ITR Team. Team members, however, acting as independent reviewers, frequently assess the information from a perspective that differs significantly from that of the Mound Plant personnel. The report is based on information obtained and conditions observed during the March 1994 review interval. The ITR process and normal site work often initiate rapid, beneficial changes in understanding and organization immediately following the review. These changes frequently alter conditions observed during the review, but the report does not address changes subsequent to the review interval.

Herein we review the technical features and challenges of putting these different technologies together. We emphasize the conceptual to make the material accessible to a wide audience. We begin by reviewing PET/CT, a more mature multi-modality imaging technology, to provide a basis for comparison to the history of PET/MR development. We discuss the motivation and challenges of PET/MR and different approaches that have been used to meet the challenges.   13. TOPICAL REVIEW: Monte Carlo modelling of external radiotherapy photon beams

    An essential requirement for successful radiation therapy is that the discrepancies between dose distributions calculated at the treatment planning stage and those delivered to the patient are minimized. An important component in the treatment planning process is the accurate calculation of dose distributions. The most accurate way to do this is by Monte Carlo calculation of particle transport, first in the geometry of the external or internal source followed by tracking the transport and energy deposition in the tissues of interest. Additionally, Monte Carlo simulations allow one to investigate the influence of source components on beams of a particular type and their contaminant particles. Since the mid 1990s, there has been an enormous increase in Monte Carlo studies dealing specifically with the subject of the present review, i.e., external photon beam Monte Carlo calculations, aided by the advent of new codes and fast computers. The foundations for this work were laid from the late 1970s until the early 1990s. In this paper we will review the progress made in this field over the last 25 years. The review will be focused mainly on Monte Carlo modelling of linear accelerator treatment heads but sections will also be devoted to kilovoltage x-ray units and 60Co teletherapy sources.

    As a result of the US Nuclear Regulatory Commission (USNRC) initiated Individual Plant Examination of External Events (IPEEE) program, virtually every operating commercial nuclear power reactor in the US has performed an assessment of severe accident risk due to external events. To date, the USNRC staff has received 63 IPEEE submittals and will receive an additional 11 by mid 1998. Currently, 49 IPEEE submittals are under various stages ore view. This paper is based on the information available for those 41 plants for which at least preliminary Technical Evaluation Reports have been prepared by the review teams. The goal of the review is to ascertain whether the licensee`s IPEEE process is capable of identifying external events-induced severe accident vulnerabilities and cost-effective safety improvements to either eliminate or reduce the impact of these vulnerabilities. The review does not, however, attempt to validate or verify the results of the licensee`s IPEEE. The primary objective of this paper is to provide an update on the preliminary perspectives and insights gained from the IPEEE process.

    A review by Catchpole et al (2009) into the causes and types of harm experienced by the surgical patient emphasised the high risk nature of the perioperative period. Investigations into recent failures at NHS organisations have emphasised the relevance of non-technical skills education in improving clinical performance and patient outcomes. However, scrub practitioner non-technical skills are often developed on a tacit basis, making assessment of performance difficult. This literature review identifies strategies that facilitate assessment of non-technical skills during surgery. Recommendations are made that will assist scrub practitioners in using a validated scrub practitioner non-technical skills assessment framework reliably. PMID:26016259

    An external peer review was conducted to review the groundwater models used in the corrective action investigation stage of the Underground Test Area (UGTA) subproject to forecast zones of potential contamination in 1,000 years for the Frenchman Flat area. The goal of the external peer review was to provide technical evaluation of the studies and to assist in assessing the readiness of the UGTA subproject to progress to monitoring activities for further model evaluation. The external peer review team consisted of six independent technical experts with expertise in geology, hydrogeology,'''groundwater modeling, and radiochemistry. The peer review team was tasked with addressing the following questions: 1. Are the modeling approaches, assumptions, and model results for Frenchman Flat consistent with the use of modeling studies as a decision tool for resolution of environmental and regulatory requirements? 2. Do the modeling results adequately account for uncertainty in models of flow and transport in the Frenchman Flat hydrological setting? a. Are the models of sufficient scale/resolution to adequately predict contaminant transport in the Frenchman Flat setting? b. Have all key processes been included in the model? c. Are the methods used to forecast contaminant boundaries from the transport modeling studies reasonable and appropriate? d. Are the assessments of uncertainty technically sound and consistent with state-of-the-art approaches currently used in the hydrological sciences? 3.     The purpose of the European Commission technical recommendations (TR) for monitoring individuals occupationally exposed to external radiation is to provide guidance on those aspects of the implementation of the European Union Parliament and Council Directives directly related to individual monitoring of external radiation, and to encourage harmonisation thereof. They are mainly aimed at the management and staff of IM services but also at manufacturers, laboratories supplying type-testing services, national authorities trying to harmonise approval procedures, and government bodies to harmonise regulations and guidance. The TR main topics are: objectives and aims of IM for external radiation; dosimetry concepts; accuracy requirements; calibration, type testing and performance testing; approval procedures; quality assurance and quality control; and dose record keeping. Attention is paid to particular aspects, such as wide energy ranges for the use of personal dosemeters, pulsed fields and non-charged particle equilibrium; and use of active personal dosemeters.     The purpose of this review is to determine the adequacy of the High Flux Beam Reactor (HFBR) Technical Specifications for 40 MW operation by comparison with the HFBR Final Safety Analysis Report, particularly the accident analyses chapter. Specifically, the Technical Specifications were compared against the Design Basis Accident (DBA) Analyses presented in the Addendum to the HFBR FSAR for 60 MW Operation. The 60 MW DBA analyses was used since it is more current and complete than the analyses presented in the original FSAR which is considered obsolete. A listing of the required systems and equipment was made for each of the accidents analyzed. Additionally, the Technical Specification instrument setpoints were compared to the DBA analyses parametric values. Also included in this review was a comparison of the Technical Specification Bases against the FSAR and the identification of any differences. The HFBR Operations Procedures Manual (OPM) was also reviewed for any inconsistencies between the FSAR or the Technical Specifications. Upon completion of this review it was determined that the Technical Specifications are well written and the items commented on should not delay the low power restart (40 MW). Additionally, the OPM is also well written and does not require further modification before restart.

    This report describes the state of practices of design reviews at NASA and research into what can be done to improve peer review practices. There are many types of reviews at NASA: required and not, formalized and informal, programmatic and technical. Standing project formal reviews such as the Preliminary Design Review and Critical Design Review are a required part of every project and mission development. However, the technical, engineering peer reviews that support teams' work on such projects are informal, some times ad hoc, and inconsistent across the organization. The goal of this work is to identify best practices and lessons learned from NASA's experience, supported by academic research and methodologies to ultimately improve the process. This research has determined that the organization, composition, scope, and approach of the reviews impact their success. Failure Modes and Effects Analysis (FMEA) can identify key areas of concern before or in the reviews. Product definition tools like the Project Priority Matrix, engineering-focused Customer Value Chain Analysis (CVCA), and project or system-based Quality Function Deployment (QFD) help prioritize resources in reviews. The use of information technology and structured design methodologies can strengthen the engineering peer review process to help NASA work towards error-proofing the design process.

    The alcohol fuels program consists of in-house and subcontracted research for the conversion of lignocellulosic biomass into fuel alcohols via thermoconversion and bioconversion technologies. In the thermoconversion area, the SERI gasifier has been operated on a one-ton per day scale and produces a clean, medium-Btu gas that can be used to manufacture methanol with a relatively small gas-water shift reaction requirement. Recent research has produced catalysts that make methanol and a mixture of higher alcohols from the biomass-derived synthetic gas. Three hydrolysis processes have emerged as candidates for more focused research. They are: a high-temperature, dilute-acid, plug-flow approach based on the Dartmouth reactor; steam explosion pretreatment followed by hydrolysis using the RUT-C30 fungal organism; and direct microbial conversion of the cellulose to ethanol using bacteria in a single or mixed culture. Modeling studies, including parametric and sensitivity analyses, have recently been completed. The results of these studies will lead to a better definition of the present state-of-the-art for these processes and provide a framework for establishing the research and process engineering issues that still need resolution. In addition to these modeling studies, economic feasibility studies are being carried out by commercial engineering firms. Their results will supplement and add commercial validity to the program results. The feasibility contractors will provide input at two levels: Technical and economic assessment of the current state-of-the-art in alcohol production from lignocellulosic biomass via thermoconversion to produce methanol and higher alcohol mixtures and bioconversion to produce ethanol; and identification of research areas having the potential to significantly reduce the cost of production of alcohols.

Results: extensive development work and scaleup has been documented on coal gasification and hazardous waste which forms a strong experience base for this MSO process; it is clearly applicable to DOE wastes such as organic liquids and low-ash wastes. It also has potential for processing difficult-to-treat wastes such as nuclear grade graphite and TBP, and it may be suitable for other problem waste streams such as sodium metal. MSO operating systems may be constructed in relatively small units for small quantity generators. Public perceptions could be favorable if acceptable performance data are presented fairly; MSO will likely require compliance with regulations for incineration.       External peer review was introduced in general hospitals in the Netherlands in 1994 to assess and improve the multidisciplinary team approach in cancer care. This paper aims to explore the value, perceived impact, and (future) role of external peer review in cancer care. Semistructured interviews were held with clinicians, oncology nurses, and managers from fifteen general hospitals that participated in three rounds of peer review over a period of 16 years. Interviewees reflected on the goals and expectations, experiences, perceived impact, and future role of external peer review. Transcriptions of the interviews were coded to discover recurrent themes. Improving clinical care and organization were the main motives for participation. Positive impact was perceived on multiple aspects of care such as shared responsibilities, internal prioritization of cancer care, improved communication, and a clear structure and position of cancer care within general hospitals. Establishing a direct relationship between the external peer review and organizational or clinical impact proved to be difficult. Criticism was raised on the content of the program being too theoretical and organization-focussed after three rounds. According to most stakeholders, external peer review can improve multidisciplinary team work in cancer care; however, the acceptance is threatened by a perceived disbalance between effort and visible clinical impact. Leaner and more clinically focused programs are needed to keep repeated peer reviews challenging and worthwhile. PMID:26714788

      The purpose of this report was to provide, at BPA's request, a technical review of interim products received for Project 2001-015-00 under contract 6925. BPA sometimes solicits technical reviews for Fish and Wildlife products or issues where outside expertise is required. External review of complex project deliverables assures BPA as a funding agency that the contractor is continuing with scientifically-credible experimental techniques envisioned in the original proposal. If the project's methodology proves feasible, there could be potential applications beyond the project area to similar situations in the Columbia Basin. The Experiment involves artificial flooding during high flow periods and a determination of the portion of the return flows that end up in the Umatilla River during low flow months and within acceptable water quality parameters (e.g., low temperature, few contaminants). Flooding could be a critical water source for aquatic organisms at times of the year when flows in the lower reaches of the Umatilla River are low and water is warmer than would be desired. The experiment was proposed to test whether 'this process, recharges the shallow aquifers of the old flood plain, for natural filtration through the alluvial soils as it returns to the Umatilla River, cleaner and cooler (about 50 degree Fahrenheit) five to six month later (about July and August) substantially cooling the river and [making it] more beneficial to anadromous [fish]'. A substantial amount of preliminary data had been collected and preliminary results were submitted in an interim report 'Echo Meadows Project Winter Artificial Recharge: Final Report for 2001 Baseline (December 2002)'. A substantial amount of addition funding was provided for the last cycle of flooding (Phases II) and final analyses of the full compliment of data collected over the life of the contract (Phase III).       Introduction Ureteral trauma is rare, accounting for less than 1% of all urologic traumas. However, a missed ureteral injury can result in significant morbidity and mortality. The purpose of this article is to review the literature since 1961 with the primary objective to present the largest medical literature review, to date, regarding ureteral trauma. Several anatomic and physiologic considerations are paramount regarding ureteral injuries management. Literature review Eighty-one articles pertaining to traumatic ureteral injuries were reviewed. Data from these studies were compiled and analyzed. The majority of the study population was young males. The proximal ureter was the most frequently injured portion. Associated injuries were present in 90.4% of patients. Admission urinalysis demonstrated hematuria in only 44.4% patients. Intravenous ureterogram (IVU) failed to diagnose ureteral injuries either upon admission or in the operating room in 42.8% of cases. Ureteroureterostomy, with or without indwelling stent, was the surgical procedure of choice for both trauma surgeons and urologists (59%). Complications occurred in 36.2% of cases. The mortality rate was 17%. Conclusion The mechanism for ureteral injuries in adults is more commonly penetrating than blunt. The upper third of the ureter is more often injured than the middle and lower thirds. Associated injuries are frequently present. CT scan and retrograde pyelography accurately identify ureteral injuries when performed together. Ureteroureterostomy, with or without indwelling stent, is the surgical procedure of choice of both trauma surgeons and urologists alike. Delay in diagnosis is correlated with a poor prognosis.     There is a critical need for standard approaches to assess, report and compare the technical performance of genome-scale differential gene expression experiments. Here we assess technical performance with a proposed standard 'dashboard' of metrics derived from analysis of external spike-in RNA control ratio mixtures. These control ratio mixtures with defined abundance ratios enable assessment of diagnostic performance of differentially expressed transcript lists, limit of detection of ratio (LODR) estimates and expression ratio variability and measurement bias. The performance metrics suite is applicable to analysis of a typical experiment, and here we also apply these metrics to evaluate technical performance among laboratories. An interlaboratory study using identical samples shared among 12 laboratories with three different measurement processes demonstrates generally consistent diagnostic power across 11 laboratories. Ratio measurement variability and bias are also comparable among laboratories for the same measurement process. We observe different biases for measurement processes using different mRNA-enrichment protocols. PMID:25254650

    I was asked to submit a response to the Protocol Review forum for the journal LabAnimal by Dr. Jerald Silverman. This forum request views and opinions on issues that are faced by research animal welfare committees. The particular issue is: Dr. John Smith, a distinguished investigator at Great Eastern University, had spent his career searching for a treatment for hearing loss. After 20 years of research, he developed a drug that potentially could improve auditory function after damage to the inner ear. The drug was administered by injection into the middle ear with the use of a fine needle inserted through the eardrum. Smith submitted a protocol to the IACUC to conduct a small study on cats, proposing to treat one group of animals with the drug and another with saline, to determine if it prevented the changes in the anatomy of the inner ear that normally occur with aging.

    Review of SRT-CMA-930039, ``Nuclear Criticality Safety Evaluation (NCSE): DWPF Melter-Batch 1,`` December 1, 1993, has been performed by the Savannah River Technical Center (SRTC) Applied Physics Group. The NCSE is a criticality assessment of the Melt Cell in the DWPF. Additionally, this pertains only to Batch 1 operation, which differs from batches to follow. Plans for subsequent batch operations call for fissile material in the Salt Cell feed-stream, which necessitates a separate criticality evaluation in the future. The NCSE under review concludes that the process is safe from criticality events, even in the event that all lithium and boron neutron poisons are lost, provided uranium enrichments are less than 40%. Furthermore, if all the lithium and as much as 98% of the boron would be lost, uranium enrichments of 100% would be allowable. After a thorough review of the NCSE, this reviewer agrees with that conclusion. This technical review consisted of: an independent check of the methods and models employed, independent calculations application of ANSI/ANS 8.1, verification of WSRC Nuclear Criticality Safety Manual({sup 2}) procedures.

    The US Department of Energy requires a periodic assessment of the Microsystems Program at Sandia National Laboratories. An external review of this program is held approximately every 18 months to 24 months. The report from the External Review Panel serves as the basis for Sandia's ''self assessment'' and is a specific deliverable of the governance contract between Lockheed Martin and the Department of Energy. The External Review of Microelectronics and Microsystems for Fiscal Year 2004 was held September 27-29, 2004 at Sandia National Laboratories, Albuquerque, NM. The external review panel consisted of experts in the fields of microelectronics, photonics and microsystems from universities, industry and other Government agencies. A complete list of the panel members is included as Appendix A of the attached report. The review assessed four areas: relevance to national needs and agency mission; quality of science, technology and engineering; performance in the operation of a major facility; and program performance management and planning. Relevance to national needs and agency mission was rated as ''outstanding''. The quality of science, technology, and engineering was rated as ''outstanding''. Operation of a major facility was rated as ''outstanding'', and the category of program performance, management, and planning was rated as ''outstanding''. Sandia's Microsystems Program thus received an overall rating of ''outstanding'' [the highest possible rating].

    [Purpose] The present study reviewed studies that examined the effects of attentional focus on balance. [Methods] Keywords such as “attentional”, “focus”, and “balance” were used to find relevant research papers in PubMed (www.ncbi.nlm.nih.gov/pubmed). Forty-five papers were found, and 18 of them were used for this study, excluding review papers and papers irrelevant to the topic of this study. [Results] Among the papers used for the review, the number of papers in which external focus produced effective outcomes was 15 (83.3%). The number of papers in which both external and internal focus produced effective outcomes was 2 (11.1%). The number of paper in which no instruction about attentional focus was effective was 1 (5.5%), and the number of papers in which internal focus was effective was zero. [Conclusion] This short review suggests clinical implications about how physical therapists can use attentional focus for balance rehabilitation of patients. Instructions about external focus of attention can generally be useful as a method to improve posture and balance control. Furthermore, the present reviews indicates that external focus of attention would be more useful in a rehabilitation stage in which the difficulty level of balance performance is gradually increased.   7. Are External Cervical Orthoses Necessary after Anterior Cervical Discectomy and Fusion: A Review of the Literature

    Introduction & Background: The use of external cervical orthosis (ECO) after anterior cervical discectomy and fusion (ACDF) varies from physician to physician due to an absence of clear guidelines. Our purpose is to evaluate and present evidence answering the question, “Does ECO after ACDF improve fusion rates?” through a literature review of current evidence for and against ECO after ACDF.  Review: A PubMed database search was conducted using specific ECO and ACDF related keywords. Our search yielded a total of 1,267 abstracts and seven relevant articles. In summary, one study provided low quality of evidence results supporting the conclusion that external bracing is not associated with improved fusion rates after ACDF.  The remaining six studies provide very low quality of evidence results; two studies concluded that external bracing after cervical procedures is not associated with improved fusion rates, one study concluded that external bracing after cervical procedures is associated with improved fusion rates, and the remaining three studies lacked sufficient evidence to draw an association between external bracing after ACDF and improved fusion rates. Conclusion: We recommend against the routine use of ECO after ACDF due to a lack of improved fusion rates associated with external bracing after surgery. PMID:27555986

    This review explores the relation of religiousness and spirituality with externalizing psychopathology in adolescence given the heightened externalizing psychopathology during this developmental period. Utilizing a developmental psychopathology framework, previous literature is reviewed focusing on the diversity of pathways from religiousness and spirituality to externalizing psychopathology at multiple levels of analysis. Moreover, the pathways considered include both intraindividual factors (e.g., self-control, monitoring, delay discounting and time orientation, and neurobiological regulatory systems) and ecological factors (e.g., intergenerational transmission, parent-child relationships, and community relationships). These pathways are explored in light of theoretical viewpoints including social control theory, divine interaction theory, and the religious ecology model. Limitations of extant work are examined, including measurement and design issues, exploration of potential negative effects of religiousness and spirituality, and bias toward Western religions. Finally, future directions of research and clinical implications are discussed. PMID:26662624

    For decades, film was used as a powerful two-dimensional (2D) dosimetry tool for radiotherapy treatment verification and quality assurance. Unlike the old silver-halide based radiographic films, radiochromic films change its color upon irradiation without the need for chemical development. Radiation dose deposited within a sensitive layer of the radiochromic film initiates polymerization of the active component, the degree of which depends on the amount of energy deposited. Response of the film to radiation is commonly expressed in terms of optical density change, which can be easily measured by any photometric device. However, a number of factors may have an impact on the signal detected by the measuring device. This review summarizes technical aspects associated with the establishment of reference radiochromic film dosimetry and its subsequent use for either clinical or research applications. PMID:27020097

    This paper describes an interface connecting InterSystems MUMPS (M/VX) to an external relational DBMS, the SYBASE Database Management System. The interface enables MUMPS to operate in a relational environment and gives the MUMPS language full access to a complete set of SQL commands. MUMPS generates SQL statements as ASCII text and sends them to the RDBMS. The RDBMS executes the statements and returns ASCII results to MUMPS. The interface suggests that the language features of MUMPS make it an attractive tool for use in the relational database environment. The approach described in this paper separates MUMPS from the relational database. Positioning the relational database outside of MUMPS promotes data sharing and permits a number of different options to be used for working with the data. Other languages like C, FORTRAN, and COBOL can access the RDBMS database. Advanced tools provided by the relational database vendor can also be used. SYBASE is an advanced high-performance transaction-oriented relational database management system for the VAX/VMS and UNIX operating systems. SYBASE is designed using a distributed open-systems architecture, and is relatively easy to interface with MUMPS.

    Nuclear space powerplants can operate at temperatures below 900 K and use stainless steel construction without a weight penalty if new radiator concepts can achieve radiator weights of 1-3 kg/m{sup 2}. Conventional tube-and-fin radiators weight about 10 kg/m{sup 2} because of heavy tube walls to prevent meteroid puncture. Radiator designs that do not require meteroid protection are possible; they operate with fluids of low vapor pressure that can be exposed directly to space in external-flow radiators. An example is the {open_quotes}rotating disk radiator{close_quotes} in which centrifugal force drives a liquid film radially outward across a thin rotating metal disk; meteroid punctures cause no loss of fluid other than from evaporation, which can be small. An even lighter concept is the liquid drop radiator in which heat is radiated directly from moving liquid drops. Such radiator concepts look practical, and they may be much easier to develop than the high-temperature, refractory-metal power systems necessitated by conventional radiators.

    In this paper, the authors present a physical model developed to simulate accurate external ventricular drain (EVD) placement with realistic haptic and visual feedbacks to serve as a platform for complete procedural training. Insertion of an EVD via ventriculostomy is a common neurosurgical procedure used to monitor intracranial pressures and/or drain CSF. Currently, realistic training tools are scarce and mainly limited to virtual reality simulation systems. The use of 3D printing technology enables the development of realistic anatomical structures and customized design for physical simulators. In this study, the authors used the advantages of 3D printing to directly build the model geometry from stealth head CT scans and build a phantom brain mold based on 3D scans of a plastinated human brain. The resultant simulator provides realistic haptic feedback during a procedure, with visualization of catheter trajectory and fluid drainage. A multiinstitutional survey was also used to prove content validity of the simulator. With minor refinement, this simulator is expected to be a cost-effective tool for training neurosurgical residents in EVD placement. PMID:26115472

    We review recent research that assesses evidence for the detection of anthropogenic and natural external influences on the climate. Externally driven climate change has been detected by a number of investigators in independent data covering many parts of the climate system, including surface temperature on global and large regional scales, ocean-heat content, atmospheric circulation, and variables of the free atmosphere, such as atmospheric temperature and tropopause height. The influence of external forcing is also clearly discernible in reconstructions of hemispheric scale temperature of the last millennium. These observed climate changes are very unlikely to be due only to natural internal climate variability, and they are consistent with the responses to anthropogenic and natural external forcing of the climate system that are simulated with climate models. The evidence indicates that natural drivers such as solar variability and volcanic activity are at most partially responsible for the large-scale temperature changes observed over the past century, and that a large fraction of the warming over the last 50 years can be attributed to greenhouse gas increases. Thus the recent research supports and strengthens the IPCC Third Assessment Report conclusion that ''most of the global warming over the past 50 years is likely due to the anthropogenic increase in greenhouse gases''.

    The US Department of Energy requires a periodic 'self assessment' of Sandia's Microsystems Program. An external panel review of this program is held approximately every 18 months, and the report from the external review panel serves as the basis for the DOE 'self assessment.' The review for this fiscal year was held on September 30-October 1, 2002 at Sandia National Laboratories, Albuquerque, NM. The panel was comprised of experts in the fields of microelectronics, photonics and microsystems from universities, industry and other Government agencies. A complete list of the panel members is shown as Appendix A to the attached report. The review assesses four areas: relevance to national needs and agency mission; quality of science technology and engineering; performance in the operation of a major facility; and program performance management and planning. Relevance to national needs and agency mission was rated as 'outstanding.' The quality of science, technology, and engineering was rated as 'outstanding.' Operation of a major facility was noted as 'outstanding,' while the category of program performance, management, and planning was rated as 'outstanding.' Sandia's Microsystems Program received an overall rating of 'outstanding' [the highest possible rating]. Advances in both experiment and theory, together with improvements in diagnostics, heating and feedback methods have led to substantial and steady progress in the understanding and stabilization of these instabilities. Many of the theory and experimental techniques and results that have been developed are useful not only for the stabilization of the resistive wall mode. They can also be used to improve the general performance of fusion confinement devices.     Background Since 2002, development assistance for health has substantially increased, especially investments for HIV, tuberculosis (TB) and malaria control. We undertook a systematic review to assess and synthesize the existing evidence in the scientific literature on the health impacts of these investments. Methods and findings We systematically searched databases for peer-reviewed and grey literature, using tailored search strategies. We screened studies for study design and relevance, using predefined inclusion criteria, and selected those that enabled us to link health outcomes or impact to increased external funding. For all included studies, we recorded dataset and study characteristics, health outcomes and impacts. We analysed the data using a causal-chain framework to develop a narrative summary of the published evidence. Thirteen articles, representing 11 individual studies set in Africa and Asia reporting impacts on HIV, tuberculosis and malaria, met the inclusion criteria. Only two of these studies documented the entire causal-chain spanning from funding to programme scale-up, to outputs, outcomes and impacts. Nonetheless, overall we find a positive correlation between consecutive steps in the causal chain, suggesting that external funds for HIV, tuberculosis and malaria programmes contributed to improved health outcomes and impact. Conclusions Despite the large number of supported programmes worldwide and despite an abundance of published studies on HIV, TB and malaria control, we identified very few eligible studies that adequately demonstrated the full process by which external funding has been translated to health impact. Most of these studies did not move beyond demonstrating statistical association, as opposed to contribution or causation. We thus recommend that funding organizations and researchers increase the emphasis on ensuring data capture along the causal pathway to demonstrate effect and contribution of external financing.     In May 2006, CH2M Hill Hanford Group, Inc. chartered an Expert Review Panel (ERP) to review the current status of the Demonstration Bulk Vitrification System (DBVS). It is the consensus of the ERP that bulk vitrification is a technology that requires further development and evaluation to determine its potential for meeting the Hanford waste stabilization mission. No fatal flaws (issues that would jeopardize the overall DBVS mission that cannot be mitigated) were found, given the current state of the project. However, a number of technical issues were found that could significantly affect the project's ability to meet its overall mission as stated in the project ''Justification of Mission Need'' document, if not satisfactorily resolved. The ERP recognizes that the project has changed from an accelerated schedule demonstration project to a formally chartered project that must be in full compliance with DOE 413.3 requirements. The perspective of the ERP presented herein, is measured against the formally chartered project as stated in the approved Justification of Mission Need document. A justification of Mission Need document was approved in July 2006 which defined the objectives for the DBVS Project. In this document, DOE concluded that bulk vitrification is a viable technology that requires additional development to determine its potential applicability to treatment of a portion of the Hanford low activity waste. The DBVS mission need statement now includes the following primary objectives: (1) process approximately 190,000 gallons of Tank S-109 waste into fifty 100 metric ton boxes of vitrified product; (2) store and dispose of these boxes at Hanford's Integrated Disposal Facility (IDF); (3) evaluate the waste form characteristics; (4) gather pilot plant operability data, and (5) develop the overall life cycle system performance of bulk vitrification and produce a comparison of the bulk vitrification process to building a second LAW Immobilization facility or other

    Environmental Management (EM-32) to provide an independent technical panel to review previous and ongoing remedial activities designed to address TCE contamination in groundwater at X-740. In 1999, a corrective action was implemented at X-740 to address contamination in the groundwater. The DOE planted poplar trees over 2.6 acres of the groundwater plume. After several years of groundwater monitoring, the results indicated that the benefits of phytoremediation were limited and insufficient to meet remedial objectives. In 2007, an alternative remedial strategy based on in situ oxidative treatment was negotiated with Ohio EPA. During FY2008, three treatment phases of chemical oxidation were completed. The general charter for the independent technical review team is to review the results of s previous and ongoing remedial actions to determine whether the current strategy will be effective in meeting the end points specified in the regulatory work plan.

    Minimally invasive surgery for liver resections has a defined role and represents an accepted alternative to open techniques for selected cases. Robotic technology can overcome some of the disadvantages of the laparoscopic technique, mainly in the most complex cases. Precise dissection and microsuturing is possible, even in narrow operative fields, allowing for a better dissection of the hepatic hilum, fine lymphadenectomy, and biliary reconstruction even with small bile ducts and easier bleeding control. This technique has the potential to allow for a greater number of major resections and difficult segmentectomies to be performed in a minimally invasive fashion. The implementation of near-infrared fluorescence with indocyanine green (ICG) also allows for a more accurate recognition of vascular and biliary anatomy. The perspectives of this kind of virtually implemented imaging are very promising and may be reflected in better outcomes. The overall data present in current literature suggests that robotic liver resections are at least comparable to both open and laparoscopic surgery in terms of perioperative and postoperative outcomes.     The Commercial Crew Program (CCP) is unique to any other program office at NASA. After the agency suffered devastating budget cuts and the Shuttle Program retired, the U.S. gave up its human spaceflight capabilities. Since 2011 the U.S. has been dependent on Russia to transport American astronauts and cargo to the International Space Station (ISS) and back. NASA adapted and formed CCP, which gives private, domestic, aerospace companies unprecedented reign over America's next ride to space. The program began back in 2010 with 5 companies and is now in the final phase of certification with 2 commercial partners. The Commercial Crew Program is made up of 7 divisions, each working rigorously with the commercial providers to complete the certification phase. One of these 7 divisions is Systems Engineering and Integration (SE&I) which is partly comprised of the Safety Technical Review Board (STRB). The STRB is primarily concerned with mitigating improbable, but catastrophic hazards. It does this by identifying, managing, and tracking these hazards in reports. With the STRB being in SE&I, it significantly contributes to the overall certification of the partners' vehicles. After the partners receive agency certification approval, they will have the capability to provide the U.S. with a reliable, safe, and cost-effective means of human spaceflight and cargo transport to the ISS and back.

    We conducted a systematic review of the literature to critically analyse the evaluation and assessment frameworks that have been applied to telemedicine systems. Subjective methods were predominantly used for technical evaluation (59 %), e.g. Likert scale. Those including objective measurements (41%) were restricted to simple metrics such as network time delays. Only three papers included a rigorous standards based objective approach. Our investigation has been unable to determine a definitive standards-based telemedicine evaluation framework that exists in the literature that may be applied systematically to assess and compare telemedicine systems. We conclude that work needs to be done to address this deficiency. We have therefore developed a framework that has been used to evaluate videoconferencing systems telemedicine applications. Our method seeks to be simple to allow relatively inexperienced users to make measurements, is objective and repeatable, is standards based, is inexpensive and requires little specialist equipment. We use the EIA 1956 broadcast test card to assess resolution, grey scale and for astigmatism. Colour discrimination is assessed with the TE 106 and Ishihara 24 colour scale chart. Network protocol analysis software is used to assess network performance (throughput, delay, jitter, packet loss). PMID:17946758

    In June 2004 the U.S. Nuclear Regulatory Commission (NRC) amended its fire protection requirements to permit existing nuclear power reactor licensees to voluntarily adopt fire protection requirements contained in National Fire Protection Association (NFPA) Standard 805. NFPA 805 is a performance-based standard for nuclear power plant fire protection that is an alternative to the deterministic, prescriptive fire protection requirements, such as 10 CFR 50 Appendix R, that was issued in 1980. One aspect of implementing NFPA 805 is that the licensee adopts the performance goals, objectives, and criteria for nuclear safety specified in the Standard. These goals, objectives, and criteria can be met through the implementation of deterministic approaches or performance-based approaches, including engineering analyses, probabilistic risk assessment, and fire modeling. Licensees voluntarily adopting the fire protection requirements in NFPA 805 must submit a license amendment request (LAR) to the NRC. The LAR provides the new proposed fire protection licensing basis, including the methodology and results of required evaluations and analyses that show how the NFPA 805 performance criteria are met. As of August 2014, licensees have submitted LARs for 26 nuclear power plants, representing 42 nuclear reactor units. Of these, 7 nuclear power plants, representing 10 nuclear reactor units, have been issued a safety evaluation (SE) by the NRC approving transition of their fire protection licensing basis to one that complies with NFPA 805. Pacific Northwest National Laboratory (PNNL) supports the NRC staff’s technical review of the LARs in the areas of fundamental fire protection, safe shutdown analysis, and Probabilistic Risk Assessment (PRA). PNNL, of course, cannot speak for the nuclear industry and its choice of implementation strategies or the NRC staff’s assessment of the approaches being taken to adopt NFPA 805.     Background Before considering whether to use a multivariable (diagnostic or prognostic) prediction model, it is essential that its performance be evaluated in data that were not used to develop the model (referred to as external validation). We critically appraised the methodological conduct and reporting of external validation studies of multivariable prediction models. Methods We conducted a systematic review of articles describing some form of external validation of one or more multivariable prediction models indexed in PubMed core clinical journals published in 2010. Study data were extracted in duplicate on design, sample size, handling of missing data, reference to the original study developing the prediction models and predictive performance measures. Results 11,826 articles were identified and 78 were included for full review, which described the evaluation of 120 prediction models. in participant data that were not used to develop the model. Thirty-three articles described both the development of a prediction model and an evaluation of its performance on a separate dataset, and 45 articles described only the evaluation of an existing published prediction model on another dataset. Fifty-seven percent of the prediction models were presented and evaluated as simplified scoring systems. Sixteen percent of articles failed to report the number of outcome events in the validation datasets. Fifty-four percent of studies made no explicit mention of missing data. Sixty-seven percent did not report evaluating model calibration whilst most studies evaluated model discrimination. It was often unclear whether the reported performance measures were for the full regression model or for the simplified models. Conclusions The vast majority of studies describing some form of external validation of a multivariable prediction model were poorly reported with key details frequently not presented.     Multisystemic therapy (MST) is effective for decreasing or preventing delinquency and other externalizing behaviors and increasing prosocial or adaptive behaviors.   10. Clinical prediction models for bronchopulmonary dysplasia: a systematic review and external validation study

    Background Bronchopulmonary dysplasia (BPD) is a common complication of preterm birth. Very different models using clinical parameters at an early postnatal age to predict BPD have been developed with little extensive quantitative validation. The objective of this study is to review and validate clinical prediction models for BPD. Methods We searched the main electronic databases and abstracts from annual meetings. The STROBE instrument was used to assess the methodological quality. External validation of the retrieved models was performed using an individual patient dataset of 3229 patients at risk for BPD. Receiver operating characteristic curves were used to assess discrimination for each model by calculating the area under the curve (AUC). Calibration was assessed for the best discriminating models by visually comparing predicted and observed BPD probabilities. Results We identified 26 clinical prediction models for BPD. Although the STROBE instrument judged the quality from moderate to excellent, only four models utilised external validation and none presented calibration of the predictive value. For 19 prediction models with variables matched to our dataset, the AUCs ranged from 0.50 to 0.76 for the outcome BPD. Only two of the five best discriminating models showed good calibration. Conclusions External validation demonstrates that, except for two promising models, most existing clinical prediction models are poor to moderate predictors for BPD. To improve the predictive accuracy and identify preterm infants for future intervention studies aiming to reduce the risk of BPD, additional variables are required. Subsequently, that model should be externally validated using a proper impact analysis before its clinical implementation. PMID:24345305

    It has been long known that patients treated with ionizing radiation carry a risk of developing a second cancer in their lifetimes. Factors contributing to the recently renewed concern about the second cancer include improved cancer survival rate, younger patient population as well as emerging treatment modalities such as intensity-modulated radiation treatment (IMRT) and proton therapy that can potentially elevate secondary exposures to healthy tissues distant from the target volume. In the past 30 years, external-beam treatment technologies have evolved significantly, and a large amount of data exist but appear to be difficult to comprehend and compare. This review article aims to provide readers with an understanding of the principles and methods related to scattered doses in radiation therapy by summarizing a large collection of dosimetry and clinical studies. Basic concepts and terminology are introduced at the beginning. That is followed by a comprehensive review of dosimetry studies for external-beam treatment modalities including classical radiation therapy, 3D-conformal x-ray therapy, intensity-modulated x-ray therapy (IMRT and tomotherapy) and proton therapy. Selected clinical data on second cancer induction among radiotherapy patients are also covered. Problems in past studies and controversial issues are discussed. The needs for future studies are presented at the end.

  17. Prevention of pin site infection in external fixation: a review of the literature.

    Pin site infections are a common complication of external fixation that places a significant burden on the patient and healthcare system. Such infections increase the number of clinic visits required during a patient's course of treatment, can result in the need for additional treatment including antibiotics and surgery, and most importantly can compromise patient outcomes should osteomyelitis or instability result from pin loosening or need for pin or complete construct removal. Factors that may influence the development of pin site infections include patient-specific risk factors, surgical technique, pin design characteristics, use of prophylactic antibiotics, and the post-operative pin care protocol including cleansing, dressing changes, and showering. Despite numerous studies that work to derive evidence-based recommendations for prevention of pin site infections, substantial controversy exists in regard to the optimal protocol. This review comprehensively evaluates the current literature to provide an overview of factors that may influence the incidence of pin site infections in patients undergoing treatment with external fixators, and concludes with a description of the preferred surgical and post-operative pin site protocols employed by the senior authors (ATF and SRR).   8. Spatiotemporal Processing in Crossmodal Interactions for Perception of the External World: A Review

    Research regarding crossmodal interactions has garnered much interest in the last few decades. A variety of studies have demonstrated that multisensory information (vision, audition, tactile sensation, and so on) can perceptually interact with each other in the spatial and temporal domains. Findings regarding crossmodal interactions in the spatiotemporal domain (i.e., motion processing) have also been reported, with updates in the last few years. In this review, we summarize past and recent findings on spatiotemporal processing in crossmodal interactions regarding perception of the external world. A traditional view regarding crossmodal interactions holds that vision is superior to audition in spatial processing, but audition is dominant over vision in temporal processing. Similarly, vision is considered to have dominant effects over the other sensory modalities (i.e., visual capture) in spatiotemporal processing. However, recent findings demonstrate that sound could have a driving effect on visual motion perception. Moreover, studies regarding perceptual associative learning reported that, after association is established between a sound sequence without spatial information and visual motion information, the sound sequence could trigger visual motion perception. Other sensory information, such as motor action or smell, has also exhibited similar driving effects on visual motion perception. Additionally, recent brain imaging studies demonstrate that similar activation patterns could be observed in several brain areas, including the motion processing areas, between spatiotemporal information from different sensory modalities. Based on these findings, we suggest that multimodal information could mutually interact in spatiotemporal processing in the percept of the external world and that common perceptual and neural underlying mechanisms would exist for spatiotemporal processing.     The dose limits for emissions from the nuclear fuel cycle were established by the Environmental Protection Agency in 40 CFR Part 190 in 1977. These limits were based on assumptions regarding the growth of nuclear power and the technical capabilities of decontamination systems as well as the then-current knowledge of atmospheric dispersion and the biological effects of ionizing radiation. In the more than thirty years since the adoption of the limits, much has changed with respect to the scale of nuclear energy deployment in the United States and the scientific knowledge associated with modeling health effects from radioactivity release. Sandia National Laboratories conducted a study to examine and understand the methodologies and technical bases of 40 CFR 190 and also to determine if the conclusions of the earlier work would be different today given the current projected growth of nuclear power and the advances in scientific understanding. This report documents the results of that work.

    Scientists have long been interested in measuring external qi (EQ or wai qi) during qigong healing, and have produced a large body of literature over the past 20 years. This paper reviews the major research on measuring EQ in China and tries to help other researchers to get a picture on what has been done so as to eliminate the simple replication of already verified results. Starting with the historical background of EQ studies in China, this paper analytically reviews the major studies of EQ effects from five different categories of detectors: 1) physical signal detectors; 2) chemical dynamics methods; 3) detectors using biological materials; 4) detectors using life sensors; and 5) detectors using the human body. The focus is on the pros and cons of each detector. These studies documented some important correlates of EQ process or qi healing, which cannot be explained by psychological effect or the known biological processes. Even though the extant literature suggests that intent plays a critical role in the effect or characteristics of EQ we know little about its role in EQ effect and its relationship with qigong healing from these experiments. These studies have confirmed the existence of measurable EQ effects from various perspectives; however, none has really revealed the primary nature of EQ or how EQ healing works. Given the fact that qigong therapy is based on the dialectic view of two interdependent spheres, while modern science and medicine is based on the reductionist view of one material world, it is recommended that future studies should use more biological or life-sensor detectors to increase our understanding of the healing potentials of qigong, instead of stay at the level of verification of signals. New methodologies, new theories, and new perspectives are urgently needed for further understanding what qigong is and how EQ healing works. PMID:15285273
