    As a general medical problem, radionuclide intakes that may cause significant health effects are uncommon events. In preparing to manage a radionuclide accident, planning is the key. The medical aspects of such an accident are only one part of the management, and a professional team approach is required. Specific priorities and sequencing are necessary in medically managing a radionuclide intake. As soon as is reasonably practical, promptly remove the victim(s) from further radionuclide, radiation field, or chemical exposure. Life and limb-saving medical aid takes precedence over ionising radiation concerns in nearly all cases. Next are the prevention and/or minimisation of internal intake of radionuclides and evaluation and control of external radionuclide contamination, followed by institution of treatment to minimise the retained radionuclide. Communication with the accident victim, and his or her family, and public affairs/media issues are important. Finally, follow-up treatment for internal intakes that may cause delayed health effects is given. PMID:14527016

  9. a Generalized Program for Internal Radionuclide Dosimetry

    The development of monoclonal antibodies specific for tumor surface antigens promises a highly specific carrier medium for delivering a tumorcidal radiation dose. Dosimetry calculations of monoclonal antibodies are made difficult, however, precisely because the focus of radioactivity is targeted for a nonstandard volume in a nonstandard geometry. This precludes straightforward application of the formalism developed for internal radionuclide dosimetry by the Medical Internal Radiation Dose Committee. A software program was written to account for the perturbations introduced by the inclusion of a tumor mass as an additional source of, and target for, radiation. The program allows the interactive development of a mathematical model to account for observed biodistribution data. The model describes the time dependence of radioactivity in each organ system that retains radiolabeled antibody, including tumor. Integration of these "time-activity" curves yield cumulative activity for each organ system identified as a 'source' of radioactivity. A Monte Carlo simulation of photon transport is then executed for each source organ to obtain the fraction of radiation energy absorbed by various 'target' organs. When combined with the cumulative activity, this absorbed fraction allows an estimate of dose to be made for each target organ. The program has been validated against ten analytic models designed to span a range of common input data types. Additionally, a performance benchmark has been defined to assess the practicality of implementing the program on different computing hardware platforms. Sources of error in the computation are elaborated on, and future directions and improvements discussed. The software presents an integrated modeling/dosimetry environment particularly suited for performing Monoclonal Antibody dosimetry. It offers a viable methodology for performing prospective treatment planning, based on extrapolation of tracer kinetic data to therapeutic levels.

    The purpose of this document is to provide information to state representatives and developers of low level radioactive waste (LLW) management facilities about the radiological, chemical, and physical characteristics of selected radionuclides and their behavior in the environment. Extensive surveys of available literature provided information for this report. Certain radionuclides may contribute significantly to the dose estimated during a radiological performance assessment analysis of an LLW disposal facility. Among these are the radionuclides listed in Title 10 of the Code of Federal Regulations Part 61.55, Tables 1 and 2 (including alpha emitting transuranics with half-lives greater than 5 years). This report discusses these radionuclides and other radionuclides that may be significant during a radiological performance assessment analysis of an LLW disposal facility. This report not only includes essential information on each radionuclide, but also incorporates waste and disposal information on the radionuclide, and behavior of the radionuclide in the environment and in the human body. Radionuclides addressed in this document include technetium-99, carbon-14, iodine-129, tritium, cesium-137, strontium-90, nickel-59, plutonium-241, nickel-63, niobium-94, cobalt-60, curium -42, americium-241, uranium-238, and neptunium-237.

  11. Hydrology and radionuclide migration program 1987 progress report

    This report presents results from the Lawrence Livermore National Laboratory's participation in the Hydrology and Radionuclide Migration Program at the Nevada Test Site (NTS) during the fiscal year 1987. The report discussed initial data from a new well (UE20n-1) drilled at the Cheshire site; presents a description of a proposed laboratory study of migration of colloids in fractured media; lists data collected during the drilling and initial sampling of UE20n-1; and describes a tentative proposal for work to be performed in FY88 by Lamont-Doherty Geological Observatory. Groundwater sampled from the new well at the Cheshire site contains tritium concentrations comparable to those measured in previous years from locations above and within the Cheshire cavity. This presence of tritium, as well as several other radionuclides, in a well 100 m away from the cavity region indicates transport of radionuclides, validates a proposed model of the flow path, and provides data on rates of groundwater flow. Previous work at the Cheshire site has shown that radionuclides are transported by colloids through fractured media. However, we have no data that can be used for predictive modeling, and existing theories are not applicable. While physical transport mechanisms of sub-micrometer colloids to defined mineral surfaces are well known, predictions based on well-defined conditions differ from experimental observations by orders of magnitude. The U.C. Berkeley group has designed a laboratory experiment to quantify colloid retention and permeability alteration by the retained colloids.

    The proposed Yucca Mountain repository is anticipated to be the first facility for long-term disposal of commercial spent nuclear fuel and high-level radioactive waste in the United States. The facility, located in the southern Nevada desert, is currently in the planning stages with initial exploratory excavations completed. It is an underground facility mined into the tuffaceous volcanic rocks that sit above the local water table. The focus of the work described in this paper is the development of radionuclide absorbers or ''getter'' materials for neptunium (Np), iodine (I), and technetium (Tc) for potential deployment in the repository. ''Getter'' materials retard the migration of radionuclides through sorption, reduction, or other chemical and physical processes, thereby slowing or preventing the release and transport of radionuclides. An overview of the objectives and approaches utilized in this work with respect to materials selection and modeling of ion ''getters'' is presented. The benefits of the ''getter'' development program to the United States Department of Energy (US DOE) are outlined.

    This Program Plan describes the field radionuclide migration studies we plan to conduct in the Climax granite at the Nevada Test Site. Laboratory support studies are included to help us understand the geochemical and hydrologic processes involved in the field. The Program Plan begins with background information (Section 1) on how this program fits into the National Waste Terminal Storage Program Plan and discusses the needs for field studies of this type. The objectives stated in Section 2 are in direct response to these needs, particularly the need to determine whether laboratory studies accurately reflect actual field conditions and the need for field testing to provide a data base for verification of hydrologic and mass transport models. The technical scope (Section 3) provides a work breakdown structure that integrates the various activities and establishes a base for the technical approach described in Section 4. Our approach combines an interactive system of field and laboratory migration experiments with the use of hydrologic models for pre-test predictions and data interpretation. Section 5 on program interfaces identifies how information will be transferred to other related DOE projects. A schedule of activities and major milestones (Section 6) and the budget necessary to meet the project objectives (Section 7) are included in the Program Plan. Sections 8 and 9 contain brief descriptions of how the technical and program controls will be established and maintained and an outline of our quality assurance program. This program plan is an initial planning document and provides a general description of activities. An Engineering Test Plan containing detailed experimental test plans, an instrumentation plan and equipment design drawings will be published as a separate document.

    The Dairy Herd Management Program has served both dairymen and veterinarians very well over the past several years under a variety of conditions. A number of veterinarians have used the Dairy Herd Management Program to provide computerized dairy record service to their clients. In many of these situations, clients have decided to purchase a computer system of their own after discovering the value of having improved, computerized dairy records. The Dairy Herd Management Program is able to efficiently handle data from large dairies without disrupting daily record-keeping routines. With this data, useful reports are generated that measure actual reproductive performance against target levels or goals. Because the Dairy Herd Management Program focuses on specific time intervals and includes data from culled cows, trends or drops in reproductive performance are more quickly detected so that corrective action can be taken to minimize economic losses. The Dairy Herd Management Program's strong points include batch entry of data, an inclusive yet flexible Vet Check List of cows to be examined, and a detailed, comprehensive Reproductive Summary report. Its major weakness is the lack of a custom report generator for specific situations or conditions. This problem is being addressed in the new version. With the improvements scheduled for the new version, the Dairy Herd Management Program should be able to meet all of the needs of dairy managers and veterinarians alike, as well as become a powerful tool for conducting dairy reproductive field trials and research.     The Program Management Tool (PMT) is a comprehensive, Web-enabled business intelligence software tool for assisting program and project managers within NASA enterprises in gathering, comprehending, and disseminating information on the progress of their programs and projects. The PMT provides planning and management support for implementing NASA programmatic and project management processes and requirements. It provides an online environment for program and line management to develop, communicate, and manage their programs, projects, and tasks in a comprehensive tool suite. The information managed by use of the PMT can include monthly reports as well as data on goals, deliverables, milestones, business processes, personnel, task plans, monthly reports, and budgetary allocations. The PMT provides an intuitive and enhanced Web interface to automate the tedious process of gathering and sharing monthly progress reports, task plans, financial data, and other information on project resources based on technical, schedule, budget, and management criteria and merits. The PMT is consistent with the latest Web standards and software practices, including the use of Extensible Markup Language (XML) for exchanging data and the WebDAV (Web Distributed Authoring and Versioning) protocol for collaborative management of documents. The PMT provides graphical displays of resource allocations in the form of bar and pie charts using Microsoft Excel Visual Basic for Application (VBA) libraries. The PMT has an extensible architecture that enables integration of PMT with other strategic-information software systems, including, for example, the Erasmus reporting system, now part of the NASA Integrated Enterprise Management Program (IEMP) tool suite, at NASA Marshall Space Flight Center (MSFC).     This report is a resource document of the methods and procedures used currently in the Data Management Program of the Marshall Islands Dose Assessment and Radioecology Project. Since 1973, over 60,000 environmental samples have been collected. Our program includes relational database design, programming and maintenance; sample and information management; sample tracking; quality control; and data entry, evaluation and reduction. The usefulness of scientific databases involves careful planning in order to fulfill the requirements of any large research program. Compilation of scientific results requires consolidation of information from several databases, and incorporation of new information as it is generated. The success in combining and organizing all radionuclide analysis, sample information and statistical results into a readily accessible form, is critical to our project.

This document describes the LBNL Issues Management Program and prescribes the process for issues identification, tracking, resolution, closure, validation, and effectiveness of corrective actions. Issues that are governed by this program include program and performance deficiencies or nonconformances that may be identified through employee discovery, internal or external oversight assessment findings, suggested process improvements and associated actions that require formal corrective action. Issues may also be identified in and/or may result in Root Cause Analysis (RCA) reports, Price Anderson Amendment Act (PAAA) reports, Occurrence Reporting and Processing System (ORPS) reports, Accident Investigation reports, assessment reports, and External Oversight reports. The scope of these issues may include issues of both high and low significance as well as adverse conditions that meet the reporting requirements of the University of California (UC) Assurance Plan for LBNL or other reporting entities (e.g., U.S. Environmental Protection Agency, U.S. Department of Energy). Issues that are found as a result of a walk-around or workspace inspection that can be immediately corrected or fixed are exempt from the requirements of this document.

    The quality assurance procedures established for the operation of the U.S. Department of Energy`s Environmental Measurements Laboratory (DOE-EML`s) Quality Assessment Program (QAP) are essentially the same as those that are in effect for any EML program involving radiometric measurements. All these programs have at their core the use of radionuclide standards for their instrument calibration. This paper focuses on EML`s approach to the acquisition, calibration and application of a wide range of radionuclide sources that are required to meet its programmatic needs.

    Having worked in the Employees and Commercial Payments Branch of the Financial Management Division for the past 3 summers, I have seen the many changes that have occurred within the NASA organization. As I return each summer, I find that new programs and systems have been adapted to better serve the needs of the Center and of the Agency. The NASA Agency has transformed itself the past couple years with the implementation of the Integrated Financial Management Program (IFMP). IFMP is designed to allow the Agency to improve its management of its Financial, Physical, and Human Resources through the use of multiple enterprise module applications. With my mentor, Joseph Kan, being the branch chief of the Employees and Commercial Payments Branch, I have been exposed to several modules, such as Travel Manager, WebTads, and Core Financial/SAP, which were implemented in the last couple of years under the IFMP. The implementation of these agency-wide systems has sometimes proven to be troublesome. Prior to IFMP, each NASA Center utilizes their own systems for Payroll, Travel, Accounts Payable, etc. But with the implementation of the Integrated Financial Management Program, all the "legacy" systems had to be eliminated. As a result, a great deal of enhancement and preparation work is necessary to ease the transformation from the old systems to the new. All this work occurs simultaneously; for example, e-Payroll will "go live" in several months, but a system like Travel Manager will need to have information upgraded within the system to meet the requirements set by Headquarters. My assignments this summer have given me the opportunity to become involved with such work. So far, I have been given the opportunity to participate in projects resulting from a congressional request, several bankcard reconciliations, updating routing lists for Travel Manager, updating the majordomo list for Travel Manager approvers and point of contacts, and a NASA Headquarters project involving

  6. PABLM: a computer program to calculate accumulated radiation doses from radionuclides in the environment

    A computer program, PABLM, was written to facilitate the calculation of internal radiation doses to man from radionuclides in food products and external radiation doses from radionuclides in the environment. This report contains details of mathematical models used and calculational procedures required to run the computer program. Radiation doses from radionuclides in the environment may be calculated from deposition on the soil or plants during an atmospheric or liquid release, or from exposure to residual radionuclides in the environment after the releases have ended. Radioactive decay is considered during the release of radionuclides, after they are deposited on the plants or ground, and during holdup of food after harvest. The radiation dose models consider several exposure pathways. Doses may be calculated for either a maximum-exposed individual or for a population group. The doses calculated are accumulated doses from continuous chronic exposure. A first-year committed dose is calculated as well as an integrated dose for a selected number of years. The equations for calculating internal radiation doses are derived from those given by the International Commission on Radiological Protection (ICRP) for body burdens and MPC's of each radionuclide. The radiation doses from external exposure to contaminated water and soil are calculated using the basic assumption that the contaminated medium is large enough to be considered an infinite volume or plane relative to the range of the emitted radiations. The equations for calculations of the radiation dose from external exposure to shoreline sediments include a correction for the finite width of the contaminated beach.

    Recent drilling affords new opportunities to investigate the occurrence, distribution and transport of radionuclides in the unsaturated and saturated zone at the Nevada Test Site (NTS), Nye County, Nevada. This program is unique becmise of the elevated activities of radionuclides encountered during drilling (> 3.7E+6 Bq/L 3H), extreme completion depths (> 950 m), the expense of constructing new wells (> $IE+6/borehole), and collaboration of government, academic, and industrial partners in the planning and execution of the program. The recent chilling is significant because it substantively augments earlier field of radionuclide migration at NTS, most notably the 1974 CAMBRIC RNM experiment Sites of five nuclear tests fired below or adjacent to the saturated zone have been drilled. Three of the events were fired in Yucca Flat which is a hydrologically closed basin and two were fired in fractured volcanics of Pahute Mesa. Results from Yucca Flat indicate that volatile and refractory radionuclides, fractionated at zero time, we not highly mobile under sawmted conditions. In contrast, borcholes completed on Pahute Mesa indicate Wgh concentrations of tritium (> 3.7E+6 Bq/L 3H) and other radionuclides may be rted more than 300 m from event cavities as dissolved species or as colloids.

    The release of radioactive material due to accidents or terroristic actions can result in radioactive contamination of the environment and may lead to intake and incorporation of radionuclides with the consequence of prolonged radiation exposure. In this case, a decision about countermeasures must be carried out as soon as possible to reduce the resulting radiation dose. In order to be prepared for such a scenario, an Assessment and Documentation System has been developed to support the rapid assessment of internal exposures and to assist in decision making. Radionuclide distributions, excretion rates, and resulting exposures have been calculated on the basis of a reference scenario. The documentation of the results in the form of tables and graphs allows the easy and quick interpretation of measurements in terms of exposure and intake. The system in its present status gives information about possibilities of countermeasures; it is the intention of the next steps of development to give advice on the basis of estimated avertable doses.   13. Sediment and Radionuclide Transport in Rivers; Field Sampling Program, Cattaraugus and Buttermilk Creeks, New York

    This report describes FY-1979 analysis results on flow, sediment and radionuclide data collected in Cattaraugus, Buttermilk and Franks Creek, New York. The objective of the study is to investigate the radionuclide transport in these streams as a part of a continuing program to provide data required for application and verification of the Sediment and Radionuclide Transport Model (SERATRA). Radiological analyses were performed on sand, silt and clay size fractions of suspended and bed sediment, as well as for dissolved radionuclides. These include gamma-ray emitters plus {sup 90}Sr, {sup 238}Pu, {sup 239 - 240}Pu, {sup 241}Am, {sup 244}Cm and {sup 3}H. Among radionuclides analyzed to date, a principal radionuclide found in the study area is {sup 137}Cs. Distributions of {sup 137}Cs associated with suspended and bed sediments have sharp peaks at the mouth of the Franks Creek, revealing the contribution of {sup 137}Cs from the NFS site. Concentration of {sup 137}Cs associated with a clay size fraction of suspended and bed sediments at the mouth of Franks Creek were 32.5 {+-} 1.5 and 134.0 {+-} 0.90 pCi/g, respectively. Cesium-134 and cobalt-60 associated with the bed sediment also have higher peaks at the mouth of Franks Creek. However, dissolved and particulate concentrations of other radionuclides analyzed under this study were generally very low and there is no clear evidence to indicate that these radionculides detected in this study area originated from the Nuclear Fuel Service site.

    Radionuclide and metal contaminants are present in the vadose zone and groundwater throughout the U.S. Department of Energy (DOE) weapons complex. In situ containment and stabilization of these contaminants in vadose zones or groundwater is a cost-effective treatment strategy. Our facilitated approach relies upon the hydrolysis of introduced urea to cause the acceleration of calcium carbonate precipitation (and trace metal coprecipitation) by increasing groundwater pH and alkalinity (Fujita et al., 2000; Warren et al., 2001). Subsurface urea hydrolysis is catalyzed by the urease enzyme, which may be either introduced with the urea or produced in situ by ubiquitous subsurface urea hydrolyzing microorganisms. Because the precipitation processes are irreversible and many western aquifers are saturated with respect to calcite, the co-precipitated metals and radionuclides will be effectively removed from groundwater. The rate at which trace metals are incorporated into calcite is a function of calcite precipitation kinetics, adsorption interactions between the calcite surface and the trace metal in solution (Zachara et al., 1991), solid solution properties of the trace metal in calcite (Tesoriero and Pankow, 1996), and also the surfaces upon which the calcite is precipitating. A fundamental understanding of the coupling of calcite precipitation and trace metal partitioning, and how this occurs in aquifers and vadose environments is lacking. This report summarizes work undertaken during the second year of this project.

    Program Managers (PM) can protect program resources and improve chances of success by anticipating, understanding and managing risks. Understanding the range of potential risks helps one to avoid or manage the risks. A PM must choose which risks to accept to reduce fire fighting, must meet the expectations of stakeholders consistently, and avoid falling into costly "black holes" that may open. A good risk management process provides the PM more confidence to seize opportunities save money, meet schedule, even improve relationships with people important to the program. Evidence of managing risk and sound internal controls can mean better support from superiors for the program by building a trust and reputation from being on top of issues. Risk managers have an obligation to provide the PM with the best information possible to allow the benefits to be realized (Small Business Consortium, 2004). The Institute for Chartered Accountants in England and Wales sees very important benefits for companies in providing better information about what they do to assess and manage key business risks. Such information will: a) provide practical forward-looking information; b) reduce the cost of capital; c) encourage better risk management; and d) improve accountability for stewardship, investor protection and the usefulness of financial reporting. We are particularly convinced that enhanced risk reporting will help listed companies obtain capital at the lowest possible cost (The Institute of Chartered Accountants in England &Wales, June 2002). Risk managers can take a significant role in quantifying the success of their department and communicating those figures to executive (program) management levels while pushing for a broader risk management role.     Radionuclide and metal contaminants are present in the vadose zone and groundwater throughout the U.S. Department of Energy (DOE) energy research and weapons complex. In situ containment and stabilization of these contaminants represents a cost-effective treatment strategy that minimizes workers’ exposure to hazardous substances, does not require removal or transport of contaminants, and generally does not generate a secondary waste stream. We have investigated an in situ bioremediation approach that immobilizes radionuclides or contaminant metals (e.g., strontium-90) by their microbially facilitated co-precipitation with calcium carbonate in groundwater and vadose zone systems. Calcite, a common mineral in many aquifers and vadose zones in the arid west, can incorporate divalent metals such as strontium, cadmium, lead, and cobalt into its crystal structure by the formation of a solid solution. Collaborative research undertaken by the Idaho National Laboratory (INL), University of Idaho, and University of Toronto as part of this Environmental Management Science Program project has focused on in situ microbially-catalyzed urea hydrolysis, which results in an increase in pH, carbonate alkalinity, ammonium, calcite precipitation, and co-precipitation of divalent cations. In calcite-saturated aquifers, microbially facilitated co-precipitation with calcium carbonate represents a potential long-term contaminant sequestration mechanism. For Area 18, the estimated inventories were 27 Ci of /sup 241/Am, 4.9 Ci of /sup 238/Pu, 150 Ci of /sup 239,240/Pu, 1.3 Ci of /sup 60/Co, 4.9 Ci of /sup 137/Cs, 13 Ci of /sup 90/Sr, 2.1 Ci of /sup 152/Eu, 1.3 Ci of /sup 154/Eu, and 1.4 Ci of /sup 155/Eu. The locations of the measurements in Area 18 were chosen by importance sampling, which permits the calculation of an estimate of the sampling error.     As part of a study on sediment and radionuclide transport in rivers, Pacific Northwest Laboratory (PNL) is investigating the effect of sediment on the transport of radionuclides in Cattaraugus and Buttermilk Creeks, New York. A source of radioactivity in these creeks is the Western New York Nuclear Service Center which consists of a low-level waste disposal site and a nuclear fuel reprocessing plant. Other sources of radioactivity include fallout from worldwide weapons testing and natural background radioactivity. The major objective of the PNL Field Sampling Program is to provide data on sediment and radionuclide characteristics in Cattaraugus and Buttermilk Creeks to verify the use of the Sediment and Radionuclide Transport model, SERATRA, for nontidal rivers. This report covers the results of field data collection conducted during September 1978. Radiological analysis of sand, silt, and clay size fractions of suspended and bed sediment, and water were performed. Results of these analyses indicate that the principal radionuclides occurring in these two water courses, with levels significantly higher than background levels, during the Phase 2 sampling program were Cesium-137 and Strontium-90. These radionuclides had significantly higher activity levels above background in the bed sediment, suspended sediment, and water samples. Other radionuclides that are possibly being released into the surface water environment by the Nuclear Fuel Services facilities are Plutonium-238, 239, and 240, Americium-241, Curium-244, and Tritium. More radionuclides were consistently found in the bed sediment as compared to suspended sediment. The fewest radionuclides were found in the water of Buttermilk and Cattaraugus Creeks. The higher levels were found in the bed sediments for the gamma-emitters and in the suspended sediment for the alpha and beta-emitters (not including Tritium).

The Uranium Mill Tailings Remedial Action (UMTRA) Project Office has prepared a ``Groundwater Protection Management Program Plan`` (groundwater protection plan) of sufficient scope and detail to reflect the program`s significance and address the seven activities required in DOE Order 5400.1, Chapter 3, for special program planning. The groundwater protection plan highlights the methods designed to preserve, protect, and monitor groundwater resources at UMTRA Project processing and disposal sites.     The objective of the Solid Waste Management Program Plan (SWMPP) is to provide a summary level comprehensive approach for the storage, treatment, and disposal of current and future solid waste received at the Hanford Site (from onsite and offsite generators) in a manner compliant with current and evolving regulations and orders (federal, state, and Westinghouse Hanford Company (Westinghouse Hanford)). The Plan also presents activities required for disposal of selected wastes currently in retrievable storage.     As government funding grows scarce, competition between the national laboratories is increasing dramatically. In this era of tougher competition, there is no for resistance to change. There must instead be a uniform commitment to improving the overall quality of our products (research and technology) and an increased focus on our customers` needs. There has been an ongoing effort to bring the principles of total quality management (TQM) to all Energy Systems employees to help them better prepare for future changes while responding to the pressures on federal budgets. The need exists for instituting a vigorous program of education and training to an understanding of the techniques needed to improve and initiate a change in organizational culture. The TQM facilitator is responsible for educating the work force on the benefits of self-managed work teams, designing a program of instruction for implementation, and thus getting TQM off the ground at the worker and first-line supervisory levels so that the benefits can flow back up. This program plan presents a conceptual model for TQM in the form of a hot air balloon. In this model, there are numerous factors which can individually and collectively impede the progress of TQM within the division and the Laboratory. When these factors are addressed and corrected, the benefits of TQM become more visible. As this occurs, it is hoped that workers and management alike will grasp the ``total quality`` concept as an acceptable agent for change and continual improvement. TQM can then rise to the occasion and take its rightful place as an integral and valid step in the Laboratory`s formula for survival.

    This report presents the results from FY 1998 technical studies conducted by Lawrence Livermore National Laboratory (LLNL) as part of the Hydrology and Radionuclide Migration Program (HRMP) and Underground Test Area (UGTA) project. The HRMP is sponsored by Defense Programs (DP) of the U.S. Department of Energy, Nevada Operations Office (DOE/NV), and supports DP operations at the Nevada Test Site (NTS) through studies of radiochemistry and resource management related to the defense programs mission.   5. Repository-Based Software Engineering Program: Working Program Management Plan

    Repository-Based Software Engineering Program (RBSE) is a National Aeronautics and Space Administration (NASA) sponsored program dedicated to introducing and supporting common, effective approaches to software engineering practices. The process of conceiving, designing, building, and maintaining software systems by using existing software assets that are stored in a specialized operational reuse library or repository, accessible to system designers, is the foundation of the program. In addition to operating a software repository, RBSE promotes (1) software engineering technology transfer, (2) academic and instructional support of reuse programs, (3) the use of common software engineering standards and practices, (4) software reuse technology research, and (5) interoperability between reuse libraries. This Program Management Plan (PMP) is intended to communicate program goals and objectives, describe major work areas, and define a management report and control process. This process will assist the Program Manager, University of Houston at Clear Lake (UHCL) in tracking work progress and describing major program activities to NASA management. The goal of this PMP is to make managing the RBSE program a relatively easy process that improves the work of all team members. The PMP describes work areas addressed and work efforts being accomplished by the program; however, it is not intended as a complete description of the program. Its focus is on providing management tools and management processes for monitoring, evaluating, and administering the program; and it includes schedules for charting milestones and deliveries of program products. The PMP was developed by soliciting and obtaining guidance from appropriate program participants, analyzing program management guidance, and reviewing related program management documents.

    Senior management of TransAlta Utilities Corp. (TAU) Calgary, Alberta, Canada, was highly dissatisfied with the fact that even though the distribution line clearance budget had been increased an average 35 percent a year between 1978 and 1984, there were few if any discernible positive results. Tree-related power outages kept increasing and budget requests from the field kept increasing. In searching for a solution TAU had to deal with the concept that the right level of funding can only be determined through an inventory of tree work. This inventory is comprised of two factors, the number of trees in proximity to the power lines and the local growth rates. Based on the inventory of a hired consultant, a 12-year budget projection was established. The period covered entailed a six-year, first-cycle or catch up phase, and a six-year, second-cycle maintenance phase. In implementing the new vegetation management program in 1986, TAU decided to contact each landowner directly to obtain consent to undertake the tree work. The intent was to reduce the risk of claims while maximizing tree removals. Complaints and claims were dramatically reduced and currently run at about one per 1,000 landowners and budgets have dropped back to the 1985 levels as predicted for the maintenance phase.

    This paper describes the management approach being taken within the Hazardous Waste Remedial Actions Program (HAZWRAP) Support Contractor Office (SCO) to ensure quality of services in a highly competitive waste management environment. An overview is presented of the contractor support role assigned to Martin Marietta Energy Systems, Inc., by the Department of Energy (DOE) national program for managing hazardous waste. The HAZWRAP SCO mission, organizational structure, and major programs are outlined, with emphasis on waste management planning for the DOE Work for Others (WFO) Program. The HAZWRAP SCO provides waste management technical support, via interagency agreements between DOE and various Department of Defense (DOD) agencies for DOD sponsors planning remedial response actions. The remainder of the paper focuses on how the concept of Total Quality Management is applied to the HAZWRAP Remedial Actions Planning (RAP) Program. The management challenge is to achieve quality on a ''system'' basis where all functional elements of program management synergistically contribute to the total quality of the effort. The quality assurance (QA) program requirements applied to the RAP Program and its subcontractors are discussed. The application of management principles in the areas of program management, procurement, and QA to achieve total quality is presented. 3 refs.

    A field sampling program was conducted on Cattaraugus and Buttermilk Creeks, New York during April 1979 to investigate the transport of radionuclides in surface waters as part of a continuing program to provide data for application and verification of Pacific Northwest Laboratory's (PNL) sediment and radionuclide transport model, SERATRA. Bed sediment, suspended sediment and water samples were collected during unsteady flow conditions over a 45 mile reach of stream channel. Radiological analysis of these samples included gamma ray spectrometry analysis, and radiochemical separation and analysis of Sr-90, Pu-238, Pu-239, 240, Am-241 and Cm-244. Tritium analysis was also performed on water samples. Based on the evaluation of radionuclide levels in Cattaraugus and Buttermilk Creeks, the Nuclear Fuel Services facility at West Valley, New York, may be the source of Cs-137, Sr-90, Cs-134, Co-60, Pu-238, Pu-239, 240, Am-241, Cm-244 and tritium found in the bed sediment, suspended sediment and water of Buttermilk and Cattaraugus Creeks. This field sampling effort was the last of a three phase program to collect hydrologic and radiologic data at different flow conditions.

  1. Clinical applications of radionuclide imaging in the evaluation and management of patients with congenital heart disease.

    Non-invasive testing of children with congenital heart disease (CHD) began in the 1950s with the introduction of radionuclide studies to assess shunt fractions, pulmonary blood flow, and ventricular contractile function. Echocardiography and cardiac magnetic resonance imaging have since replaced radionuclide imaging in many of these roles. Concurrently, percutaneous and surgical repairs of complex CHD evolved, creating new roles for radionuclide imaging. In this paper on applications of radionuclide imaging in CHD, we review the multiple mechanisms for myocardial ischemia in CHD. We critically compare optimal radionuclide imaging techniques to other imaging modalities for assessing ischemia in CHD. We present the current role of nuclear imaging for assessing viability and pulmonary blood flow. We highlight the value added by advances in dedicated cardiac SPECT scanners, novel reconstruction software, and cardiac PET in performing low-dose radionuclide imaging in CHD. Finally, we discuss the emerging clinical indications for radionuclide imaging in CHD including coronary flow reserve assessment and evaluation of cardiovascular prosthesis and device infections.     Sodium oxybate, also known as gamma-hydroxybutyric acid (GHB), was discovered in 1960 and has been described both as a therapeutic agent with high medical value and, more recently, a substance of abuse. The naturally occurring form of this drug is found in various body tissues but has been studied most extensively in the CNS where its possible function as a neurotransmitter continues to be studied. Sodium oxybate has been approved in different countries for such varied uses as general anaesthesia, the treatment of alcohol withdrawal and addiction, and, most recently, cataplexy associated with narcolepsy. During the 1980s, easy access to GHB-containing products led to various unapproved uses, including weight loss, bodybuilding and the treatment of sleeplessness, sometimes with serious long-term effects. The availability of these unapproved and unregulated forms of the drug led to GHB and its analogues being popularised as substances of abuse and subsequent notoriety as agents used in drug-facilitated sexual assault, or 'date rape', eventually leading to the prohibition of GHB sales in the US. Legal efforts to control the sale and distribution of GHB and its analogues nearly prevented the clinical development of sodium oxybate for narcolepsy in the US. However, following extensive discussions with a variety of interested parties, a satisfactory solution was devised, including legislative action and the development of the Xyrem Risk Management Program. Amendments to the US Controlled Substances Act made GHB a schedule I drug, but also contained provisions that allow US FDA-approved products to be placed under schedule III. This unique, bifurcated schedule for sodium oxybate/GHB allowed the clinical development of sodium oxybate to proceed and, in July 2002, it was approved by the FDA as an orphan drug for the treatment of cataplexy in patients with narcolepsy as Xyrem(sodium oxybate) oral solution.     Potential radionuclide migration in geologic repositories is being addressed. Geohydrologic settings being investigated include unsaturated tuff, saturated basalt, and bedded salt. Several approaches to assessing potential migration are being considered. Mass transfer is prominent among near-field approaches, involving estimation of diffusive transport rates through the engineered barrier system using analytical and finite element methods. For far-field analysis of migration in the geosphere, detailed characterizations of potential repository sites will lead to site-specific models describing radionuclide migration for a variety of postulated release scenarios. Finite-element and finite-difference codes are being used and developed to solve the mathematical equations pertinent to far-field assessments. Computational approaches presently in use generally require distribution coefficients to estimate the retardation of specific radionuclides with respect to the transport rate of water.

    Chronic disease management has been a rapidly growing entity in the 21st century as a strategy for managing chronic illnesses in large populations. However, experience has shown that disease management programs have not been able to demonstrate their financial value. The objectives of disease management programs are to create quality benchmarks, such as principles and guidelines, and to establish a uniform set of metrics and a standardized methodology for evaluating them. In order to illuminate the essence of disease management and its components, as well as the complexity and the problematic nature of performing economic calculations of their profitability and value, we collected data from several reports that dealt with the economic intervention of disease management programs. The disease management economic evaluation is composed of a series of steps, including the following major categories: data/information technology, information generation, assessment/recommendations, actionable customer plans, and program assessment/reassessment. We demonstrate the elements necessary for economic analysis. Disease management is one of the most innovative tools in the managed care environment and is still in the process of being defined. Therefore, objectives should include the creation of quality measures, such as principles and guidelines, and the establishment of a uniform set of metrics and a standardized methodology for evaluating them.     Disease management programs offer the benefits of lower disease occurrence, improved patient care, and lower healthcare costs. In such programs, the key mechanism used to identify individuals at risk for targeted diseases is the data warehouse. This article surveys recent warehousing techniques from HMOs to map out critical issues relating to the preparation, design, and implementation of a successful data warehouse. Discussions of scope, data cleansing, and storage management are included in depicting warehouse preparation and design; data implementation options are contrasted. Examples are provided of data warehouse execution in disease management programs that identify members with preexisting illnesses, as well as those exhibiting high-risk conditions. The proper deployment of successful data warehouses in disease management programs benefits both the organization and the member. Organizations benefit from decreased medical costs; members benefit through an improved quality of life through disease-specific care. PMID:11452582

    The Radioactive Waste Management Complex (RWMC) Site Characterization Program is a continuation of the Subsurface Investigation Program (SIP). The scope of the SIP has broadened in response to the results of past work that identified hazardous as well as radionuclide contaminants in the subsurface environment and in response to the need to meet regulatory requirements. Two deep boreholes were cored at the RWMC during FY-1988. Selected sediment samples were submitted for Appendix IX of 40 CFR Part 264 and radionuclide analyses. Detailed geologic logging of archived core was initiated. Stratigraphic studies of the unsaturated zone were conducted. Studies to determine hydrologic properties of sediments and basalts were conducted. Geochemical studies and analyses were initiated to evaluate contaminant and radionuclide speciation and migration in the Subsurface Disposal Area (SDA) geochemical environment. Analyses of interbed sediments in boreholes D15 and 8801D did not confirm the presence of radionuclide contamination in the 240-ft interbed. Analyses of subsurface air and groundwater samples identified five volatile organic compounds of concern: carbon tetrachloride, trichloroethylene, 1,1,1-trichloroethane, chloroform, and tetrachloroethylene. 33 refs., 5 figs., 2 tabs.

    The Nevada National Security Site (NNSS), formerly the Nevada Test Site, was established in 1951 mainly for weapons testing; because special nuclear materials (SNM) were expended during the tests, a nuclear material management program was not required. That changed in December 2004 with the receipt of Category I SNM for purposes other than weapons testing. At that time, Material Control and Accountability and Nuclear Material Management were a joint laboratory (Los Alamos and Lawrence Livermore) effort with nuclear material management being performed at the laboratories. That changed in March 2006 when the U.S. Department of Energy, National Nuclear Security Administration Nevada Site Office appointed sole responsibility to the Management and Operations (M&O) contractor, National Security Technologies, LLC (NSTec). Since 2006 the basic nuclear material management work was completed by a combination of M&O employees and subcontractors, but a true Nuclear Material Management (NMM) Program was not determined to be necessary until recently. With expanding missions and more nuclear material (NM) coming to the NNSS, it became imperative to have an organization to manage these materials; therefore, an NMM Manager was officially appointed by NSTec in 2012. In June 2011 a Gap Analysis and white paper was completed by a subcontractor; this presentation will include highlights from those documents along with our plans to resolve the “gaps” and stand up a functional and compliant NMM Program at the NNSS.

    Emergency response software is used increasingly by the US Department of Energy's (DOE's) Emergency Management Project (EMP) personnel at Hanford Site. This software must be reliable, of high quality, and capable of performing critical functions to support assessment of actual or potential consequences of any hazardous accidents onsite or events having potential offsite impacts. To better control the software and ensure its suitability for use as a tool to protect employees, the public, and environment, a method for specifying and certifying its capabilities and documenting its development and implementation was needed. A team of EMP staff, composed of personnel from Pacific Northwest Laboratory (PNL) and Boeing Computer Services- Richland (BCSR) under the direction of PNL EMP, responded to this need by developing a software configuration management program (CMP). This report documents the development of the CMP, including the strategies upon which the CMP is based, and describes the program as it has been implemented for EMS System software. The program relies on the integration of its three primary elements: the configuration management staff, tools, and process. Configuration management staff run the program, using specially designed configuration management forms to guide, document, and track the life cycle of the software. The configuration management process itself is reflected in the instructive forms and summarized in flowcharts representing each phase of the process -- from requirements specification through implementation and maintenance. 7 refs., 9 figs., 1 tab.

    Radionuclide and metal contaminants such as 90Sr are present beneath U.S. Department of Energy (DOE) lands in both the groundwater (e.g., 100-N area at Hanford, WA) and vadose zone (e.g., Idaho Nuclear Technology and Engineering Center at the Idaho National Engineering and Environmental Laboratory). In situ containment and stabilization of these contaminants is a cost-effective treatment strategy. However, implementing in situ containment and stabilization approaches requires definition of the mechanisms that control contaminant sequestration. We are investigating the in situ immobilization of radionuclides or contaminant metals (e.g., 90Sr) by their facilitated co-precipitation with calcium carbonate in groundwater and vadose zone systems. Our facilitated approach, shown schematically in Figure 1, relies upon the hydrolysis of introduced urea to cause the acceleration of calcium carbonate precipitation (and trace metal co-precipitation) by increasing pH and alkalinity. Subsurface urea hydrolysis is catalyzed by the urease enzyme, which may be either introduced with the urea or produced in situ by ubiquitous subsurface urea hydrolyzing microorganisms. Because the precipitation process tends to be irreversible and many western aquifers are saturated with respect to calcite, the coprecipitated metals and radionuclides will be effectively removed from the aqueous phase over the long-term. Another advantage of the ureolysis approach is that the ammonium ions produced by the reaction can exchange with radionuclides sorbed to subsurface minerals, thereby enhancing the availability of the radionuclides for re-capture in a more stable solid phase (co-precipitation rather than adsorption).

    As NASA gears up for another attempt at redesigning Space Station Freedom, some in Congress are wondering whether the space agency has learned any lessons from a number of costly past mistakes. Louis Stokes (D-Ohio), the new chairman of the House Appropriations Veterans Affairs, Housing and Urban Development, and Independent Agencies Subcommittee, held a hearing on March 17 to examine unanticipated cost growth in a variety of projects, including the space toilet, the advanced turbo pump for the shuttle, and the Mars Observer, as well as the space station. Stokes seemed well-suited to this oversight role, asking well-informed and probing questions rather than accusatory ones. The witnesses, NASA head Daniel Goldin and many of his top managers (most of whom were not in their present positions when the projects were initiated), analyzed past errors and offered useful measures for avoiding similar problems in the future.

    The management of neuroendocrine tumours is challenging when curative surgery is ruled out because of distant metastases. We report a case of gastrointestinal carcinoid with bilateral ovarian metastases in a 50-year-old female who received octreotide therapy followed by peptide receptor radionuclide therapy and surgery thereafter. Somatostatin receptor expression on neuroendocrine tumours has implications in diagnosis and therapy. (68)Ga-DOTA-NOC PET is a recent advancement in the field of somatostatin receptor imaging. The lesions which demonstrate tracer uptake on positron emission tomographic studies can be further planned for treatment with octreotide and (177)Lu-DOTA-TATE.     The 21 Space Grant Programs represent a broad range of organizational structures which operate programs ranging in size from single university organizations to organizations including up to 41 members involving a composite of industrial organizations such as state agencies, and universities. Some of the space grant awards were made to organizations already in existence with on-going programs while other awards were made to consortia newly formed for the purpose of applying to the Space Grant Program. The workshop on organization and management of Space Grant Programs provided an opportunity for directors and program representatives to discuss and compare the relative advantages and disadvantages of the various models being used. This paper offers examples of the diversity of organizations, summarizes the common concerns to be met by each organizational model, and provides a case study of the Texas Space Grant Consortium organization.

    This report highlights the results of FY 2000 technical studies conducted by the Analytical and Nuclear Chemistry Division (ANCD) at Lawrence Livermore National Laboratory (LLNL) in support of the Hydrology and Radionuclide Migration Program (HRMP) and Underground Test Area (UGTA) Project. This is the latest in a series of annual reports published by LLNL-ANCD to document recent investigations of radionuclide migration and transport processes at the Nevada Test Site (NTS). The HRMP is sponsored by Defense Programs (DP) at the U.S. Department of Energy, Nevada Operations Office (DOENV), and supports DP operations at the NTS through studies of radiochemical and hydrologic processes that are relevant to the DP mission. Other organizations that support the HRMP include Los Alamos National Laboratory (LANL), the U.S. Geological Survey (USGS), the Desert Research Institute (DRI) of the University of Nevada, the U.S. Environmental Protection Agency (EPS), and Bechtel Nevada (BN).     The computer programs ARRRG and FOOD were written to facilitate the calculation of internal radiation doses to man from the radionuclides in the environment and external radiation doses from radionuclides in the environment. Using ARRRG, radiation doses to man may be calculated for radionuclides released to bodies of water from which people might obtain fish, other aquatic foods, or drinking water, and in which they might fish, swim or boat. With the FOOD program, radiation doses to man may be calculated from deposition on farm or garden soil and crops during either an atmospheric or water release of radionuclides. Deposition may be either directly from the air or from irrigation water. Fifteen crop or animal product pathways may be chosen. ARRAG and FOOD doses may be calculated for either a maximum-exposed individual or for a population group. Doses calculated are a one-year dose and a committed dose from one year of exposure. The exposure is usually considered as chronic; however, equations are included to calculate dose and dose commitment from acute (one-time) exposure. The equations for calculating internal dose and dose commitment are derived from those given by the International Commission on Radiological Protection (ICRP) for body burdens and Maximum Permissible Concentration (MPC) of each radionuclide. The radiation doses from external exposure to contaminated farm fields or shorelines are calculated assuming an infinite flat plane source of radionuclides. A factor of two is included for surface roughness. A modifying factor to compensate for finite extent is included in the shoreline calculations.

    The physiographic setting of Kerala State, India, is unique. A narrow strip of the state contains a chain of lagoons and estuaries with a very high population density. The strip is subjected to severe coastal erosion during the monsoon season. A number of other problems are also associated with the coastal zone of Kerala, such as irregular dredging of black sands from the beaches, coastal flooding, hazards due to developmental activities, etc. A Coastal Zone Management Program was developed and administered by the Centre for Earth Science Studies, Trivandrum, to provide efficient coastal management and solve some of these problems. Various programs included under the Coastal Zone Management are the following: (1) Sedimentological, bathymetric, and geochemical studies of lagoons and estuaries; (2) monitoring of planimetric changes of beaches by profiling beaches during different seasons all along the coast; (3) studies of the nature, distribution, and provenance of black sand deposits from beaches; (4) studies of the peculiar occurrence of patchy, calm, turbid areas of water in the offshore containing high suspended sediment concentrate known as mud banks; (5) wave studies involving continuous monitoring of wave data all along the coast in order to understand wave climate and erosion; (6) sediment movement studies using fluorescent tracer to aid in the development of ports and harbors; (7) studies on various aspects of offshore. The outlines of the various programs discussed in this article will help other states and countries to develop a coastal zone management program according to the needs of the state or country and the nature of the problem occurring in the coastal zone.

  12. 1998 Environmental Management Science Program Annual Report

    The Environmental Management Science Program (EMSP) is a collaborative partnership between the DOE Office of Environmental Management (EM), Office of Science (DOE-SC), and the Idaho Operations Office (DOE-ID) to sponsor basic environmental and waste management related research. Results are expected to lead to reduction of the costs, schedule, and risks associated with cleaning up the nation's nuclear complex. The EMSP research portfolio addresses the most challenging technical problems of the EM program related to high level waste, spent nuclear fuel, mixed waste, nuclear materials, remedial action, decontamination and decommissioning, and health, ecology, or risk. The EMSP was established in response to a mandate from Congress in the fiscal year 1996 Energy and Water Development Appropriations Act. Congress directed the Department to ''provide sufficient attention and resources to longer-term basic science research which needs to be done to ultimately reduce cleanup costs, develop a program that takes advantage of laboratory and university expertise, and seek new and innovative cleanup methods to replace current conventional approaches which are often costly and ineffective''. This mandate followed similar recommendations from the Galvin Commission to the Secretary of Energy Advisory Board. The EMSP also responds to needs identified by National Academy of Sciences experts, regulators, citizen advisory groups, and other stakeholders.

    The management and control of a representative, highly integrated high-technology project, in the X-29A aircraft flight test project is addressed. The X-29A research aircraft required the development and integration of eight distinct technologies in one aircraft. The project management system developed for the X-29A flight test program focuses on the dynamic interactions and the the intercommunication among components of the system. The insights gained from the new conceptual framework permitted subordination of departments to more functional units of decisionmaking, information processing, and communication networks. These processes were used to develop a project management system for the X-29A around the information flows that minimized the effects inherent in sampled-data systems and exploited the closed-loop multivariable nature of highly integrated projects.

    The MLF since its inception in 1996 has endeavored to develop a coordinated approach towards the improved care and treatment of liver diseases in Malaysia. Its close liaison with the Malaysian MOH, local medical associations, and corporate bodies has contributed to the success of its many programs. Educating the public, research, and training have been important elements of successful hepatitis disease control programs. Hepatitis Days have been proven to be very successful in raising the awareness of the general public to hepatitis disease. Rapid screening and vaccination has also helped to remove the social stigma associated with the disease, eliminated the need for numerous clinic appointments, and rendered vaccination more accessible to the public. The MLF perspective emphasizes the need for collaborative effort between Government bodies and other agencies, such as non-governmental organizations, laboratories, and the medical fraternity, to ensure the overall success of hepatitis disease management programs.     The Isotope Production Facility (IPF) at Los Alamos is operated on the authorization basis of a radiological facility with an inventory limit of a Category 3 Nuclear Facility. For the commissioning of IPF, a ''dummy'' target stack containing Zn, Nb and Al disks, and a ''prototype'' stack were irradiated with a proton beam. The ''prototype'' stack contained two pressed RbCl disks, encapsulated in stainless steel, and a Ga metal target. Typical ''prototype'' stack beam parameters were 88.9 {micro}A, 101.3 h. Operation procedures require the projection of all generated radionuclide activities. This is mandatory in order to determine both maximum beam current and maximum beam exposure time. The Monte Carlo code MCNPX and the burn-up code CINDER90 were used to determine maximum beam parameters prior to irradiation. After irradiation, activity estimates were calculated assuming actual average beam parameters. They were entered into an online inventory database, and were later, after chemical separation and radioactive assays, replaced by experimental values. A comparison of ''prototype'' stack experimental yield data to Monte Carlo calculation results showed that the computer codes provide realistic, conservative estimates.

  16. National NIF Diagnostic Program Interim Management Plan

    The National Ignition Facility (NIF) has the mission of supporting Stockpile Stewardship and Basic Science research in high-energy-density plasmas. To execute those missions, the facility must provide diagnostic instrumentation capable of observing and resolving in time events and radiation emissions characteristic of the plasmas of interest. The diagnostic instrumentation must conform to high standards of operability and reliability within the NIF environment. These exacting standards, together with the facility mission of supporting a diverse user base, has led to the need for a central organization charged with delivering diagnostic capability to the NIF. The National NIF Diagnostics Program (NNDP) has been set up under the aegis of the NIF Director to provide that organization authority and accountability to the wide user community for NIF. The funds necessary to perform the work of developing diagnostics for NIF will be allocated from the National NIF Diagnostics Program to the participating laboratories and organizations. The participating laboratories and organizations will design, build, and commission the diagnostics for NIF. Restricted availability of funding has had an adverse impact, unforeseen at the time of the original decision to projectize NIF Core Diagnostics Systems and Cryogenic Target Handing Systems, on the planning and initiation of these efforts. The purpose of this document is to provide an interim project management plan describing the organizational structure and management processes currently in place for NIF Core Diagnostics Systems. Preparation of a Program Execution Plan for NIF Core Diagnostics Systems has been initiated and a current draft is provided as Attachment 1 to this document.     Approximately one-half of all patients with HIV infection who are under care have Medicaid as the third party payor. Unlike Medicare, Medicaid is a state-specific program that has huge variations in reimbursement strategies. Multiple studies have shown that care for persons with AIDS is about $20,000/year, but reimbursement through various state Medicaid programs varies about $100/m/m to $2800/m/m despite the fact that expectations for care are identical. Hopkins has a major commitment to persons with HIV infection with a program that now includes 30 faculty members and a support staff of 170. With the introduction of mandatory managed care for Medicaid recipients in July, 1997, we were confronted with the issue of substantial downsizing with abandonment of over half of our patients, or learning the transition to managed care. This has been a steep learning curve involving negotiations with the state Medicaid office, reorganization of our clinic, careful scrutiny of our database regarding resource utilization and cost, education of providers, and longitudinal collection of new information and integration of the rapid changes in the field. In the process of this transition, we learned that there are precious few resources to provide guidance and that there is a perceived need for assistance by HIV providers throughout the country. Consequently, we have now established the "HIV Managed Care Network" with substantial funding from diverse sources to support education, data collection, and public policy review. It is premature to evaluate performance since most of these activities have just begun, but we expect that this Network will serve as a demonstration model for methods to deal with chronic diseases under managed care.     Degradation of air quality in Asia due to economic growth during Past two decades is evident. The World Bank with its new mandate for sustainable development is working with Asian Governments to reverse the trend. This paper briefly describes initiatives called URBAIR and RAINS-ASIA. Urban Air Quality Strategy in Asia (URBAIR) program seeks to assist governments and the private sector in developing tools to identify immediate and long-term actions to restore air quality in Asian cities. The URBAIR program, conducted for four selected cities (Bombay, Jakarta, Kathmandu, Manila). Since air pollution management in Asian cities is in a preliminary stage, there is need to develop a comprehensive Action Plan as a framework for preparing the Air Quality Management Strategy in each city. In preparing this Action Plan, URBAIR has identified the main aspects of air pollution in the cities. These include major pollutants, their sources, and their health and economic impacts. Air quality models have been evaluated and adapted to the data available for these cities, and monitoring needs identified. An Action Plan has been proposed by the city based working groups for a comprehensive Air Quality Management Strategy.

    The main purpose of this training program element is to familiarize personnel involved in hazardous waste management with the goals of RCRA and how they are to be achieved. These goals include: to protect health and the environment; to conserve valuable material and energy resources; to prohibit future open dumping on the land; to assure that hazardous waste management practices are conducted in a manner which protects human health and the environment; to insure that hazardous waste is properly managed thereby reducing the need for corrective actions in the future; to establish a national policy to reduce or eliminate the generation of hazardous waste, wherever feasible. Another objective of this progam element is to present a brief overview of the RCRA regulations and how they are implemented/enforced by the Environmental Protection Agency (EPA) and each of the fifty states. This element also discusses where the RCRA regulations are published and how they are updated. In addition it details who is responsible for compliance with the regulations.     This report presents the results of FY 1995 technical studies conducted by the Lawrence Livermore National Laboratory (LLNL) as part of the Hydrology and Radionuclide Migration Program (HRMP), a multi-agency program sponsored by the U.S. Department of Energy, Nevada Operations Office (DOE/NV), to address the environmental consequences of nuclear weapons testing at the Nevada Test Site (NTS). A priority is to better characterize the complex near-field environment in order to assess and predict the movement of radionuclides in groundwater. Other participating organizations include the Los Alamos National Laboratory (LANL), the U.S. Geological Survey (USGS) and the Desert Research Institute (DRI) of the University of Nevada. A radiologic source term in excess of 10{sup 8} curies of tritium, fission products, activation products and actinides is residual from more than three decades of underground nuclear weapons testing at the Nevada Test Site (NTS). Burial depths to insure containment of these explosions necessitated firing approximately one third of the more than 800 underground nuclear tests within one cavity radius or below the static water table. Work at LLNL has focused on studies of radionuclide transport under saturated, partially saturated or unsaturated conditions as well as investigations of the stable, radiogenic and cosmogenic isotope systematics of NTS groundwaters. LLNL has prioritized these studies because of the significance for potential radionuclide migration at the Nevada Test Site. LLNL utilizes expertise in nuclear weapons testing, radiochemical diagnostics, nuclear test phenomenology, mass spectrometry, aqueous geochemistry and field and laboratory studies of radionuclide migration to bring a unique measurement and interpretative capability to this research.

    Underground nuclear tests conducted at the Nevada Test Site (NTS) are characterized by abundant fission product and actinide source terms. Included are {sup 99}Tc and other soluble radionuclides ({sup 3}H, {sup 14}C, {sup 36}Cl, {sup 85}Kr, and {sup 129}I), which are presumably mobile in groundwater and potentially toxic to down-gradient receptors. NTS provides the Office of Civilian Radioactive Waste Management (OCRWM) with an analog of the release of these radionuclides from a nuclear waste repository in the absence of engineered barriers. The investigation described in this report synthesizes a substantial body of data collected on the identity and distribution of soluble radionuclides at field scales over distances of hundreds of meters, for durations up to 40 years, and under hydrogeologic conditions very similar to the proposed geological repository at Yucca Mountain. This body of data is complemented by laboratory transport studies and a synthesis of recent modeling investigations from the NTS, with an emphasis on the ongoing Yucca Mountain Program (YMP) efforts. Overall, understanding the controls of radionuclide mobility associated with these nuclear tests will provide insight into the repository's future performance as well as bounds and calibrations for the numerical predictions of long-term radionuclide releases and migration.

    The Watershed and River System Management Program (WaRSMP), a joint effort between the U.S. Geological Survey (USGS) and the U.S. Bureau of Reclamation (Reclamation), is focused on research and development of decision support systems and their application to achieve an equitable balance among diverse water resource management demands. Considerations include: (1) legal and political constraints; (2) stake holder and consensus-building; (3) sound technical knowledge; (4) flood control, consumptive use, and hydropower; (5) water transfers; (6) irrigation return flows and water quality; (7) recreation; (8) habitat for endangered species; (9) water supply and proration; (10) near-surface groundwater; and (11) water ownership, accounting, and rights. To address the interdisciplinary and multi-stake holder needs of real-time watershed management, WaRSMP has developed a decision support system toolbox. The USGS Object User Interface facilitates the coupling of Reclamation's RiverWare reservoir operations model with the USGS Modular Modeling and Precipitation Runoff Modeling Systems through a central database. This integration is accomplished through the use of Model and Data Management Interfaces. WaRSMP applications include Colorado River Main stem and Gunnison Basin, the Yakima Basin, the Middle Rio Grande Basin, the Truckee-Carson Basin, and the Umatilla Basin.

    The success of the Virginia Division of Forensic Science's DNA program is based upon managing the DNA program from a central location, consistency in the methods and procedures used, cooperation and continual dedication of the laboratory staff, as well as overseeing of all technical issues by a single technical leader. To streamline the analysis of the evidence samples, the most probative items of evidence are examined first for biological materials. Once biological materials are identified, DNA analysis is performed and the resulting foreign short tandem repeat profile is entered into the Combined DNA Index Systems for routine searching of the profile against Virginia's and the nation's convicted offender DNA Data Banks. In addition, the foreign profile is also searched against other foreign profiles obtained from previously analyzed evidence samples, thereby helping the law enforcement agencies in Virginia solve crimes that would otherwise go unsolved.     RadICalc is a cross-platform program designed to calculate the intensity of radiation released by the decay of arbitrary isotopic mixtures. It was developed to address the need for a program that could calculate the composition, activity, and measurable radiation of a sample over time without significant effort from end-users. RadICalc uses Bateman's solutions for radioactive decay to determine activity over time. Radiation intensities are subsequently calculated using a database containing information about alpha particles, beta electrons, gamma- rays, conversion and Auger electrons, and X-rays. The user interface accepts input for isotopic mixture, initial number of atoms, and time passed since sample composition was known. Results are plotted graphically, and there is a search interface provided to and isotopes of interest. RadICalc can determine activity and radiation expected at specific masses with user-defined molecules in addition to atomic species; the latter is useful in mass based isotope separations for radiometric counting applications, a novel method under development at PNNL.

  18. The Oak Ridge Refrigerant Management Program

    For many years, chlorofluorocarbons (CFC's) have been used by the Department of Energy's (DOE) Oak Ridge Y-12 Plant in air conditioning and process refrigeration systems. However, Title 6 of the Clean Air Act Amendments (CAAA) and Executive Order 12843 (Procurement Requirements and Policies for Federal Agencies for Ozone Depleting Substances) signed by President Clinton require, as policy, that all federal agencies maximize their use of safe, alternate refrigerants and minimize, where economically practical, the use of Class 1 refrigerants. Unfortunately, many government facilities and industrial plants have no plan or strategy in place to make this changeover, even though their air conditioning and process refrigeration equipment may not be sustainable after CFC production ends December 31, 1995. The Y-12 Plant in Oak Ridge, Tennessee, has taken an aggressive approach to complying with the CAAA and is working with private industry and other government agencies to solve tough manufacturing and application problems associated with CFC and hydrochlorofluorocarbon (HCFC) alternatives. Y-12 was the first DOE Defense Program (DP) facility to develop a long-range Stratospheric Ozone Protection Plan for refrigerant management for compliance with the CAAA. It was also the first DOE DP facility to complete detailed engineering studies on retrofitting and replacing all air conditioning and process refrigeration equipment to enable operation with alternate refrigerants. The management plan and engineering studies are models for use by other government agencies, manufacturing plants, and private industry. This presentation identifies some of the hidden pitfalls to be encountered in the accelerated phaseout schedule of CFC's and explains how to overcome and prevent these problems. In addition, it outlines the general issues that must be considered when addressing the phase-out of ozone depleting substances and gives some 'lessons learned' by Y-12 from its Refrigerant Management

    Recently, the Diabetes Control and Complication Trial (DCCT) and other similar studies have demonstrated that near-normalization of blood glucose in diabetes will reduce complications up to 75% but translation of these results into practice has been difficult. In an attempt to help provide the best possible control of patients with diabetes, we have produced an attempt to help provide the best possible control of patients with diabetes, we have produced a new disease state management system for diabetes, called "Staged Diabetes Management" (SDM), implemented it in over 100 sites worldwide, and developed a computer program to simplify its use. SDM, designed to change the way we deal with patients with diabetes, is based upon five principles: (1) community involvement in setting care guidelines; (2) negotiation of goals with patients; (3) appropriate timelines for therapeutic success; (4) use of flowcharts for medical decisions; and (5) evaluation of the program. SDM is designed to be altered by a community to meet its needs and resources. It encourages primary care physicians to deliver better diabetes care using a team approach and to refer patients with diabetes to specialists when appropriate. It has a complete set of materials for communities, individual health care providers and patients. SDM has been tested for changes in structure, process and outcomes. A meta-analysis of seven clinical trials with over 500 patients has shown a time-weighted average fall in hemoglobin A1c of 1.7 points (equivalent to a drop in mean blood glucose of about 3.5 mM or 60 mg/dL). Preliminary pharmacoeconomic analysis demonstrates a lifetime cost saving of over $27,000 per patient. A computer program has been developed for the Microsoft Windows environment that contains a client-server database, based upon DiabCare, for the data file structure. PMID:9571514

    A program was established for the DOE Office of Environmental Management (EM) to evaluate factors that are anticipated to affect waste glass reaction during repository disposal, especially in an unsaturated environment typical of what may be expected for the proposed Yucca Mountain repository site. This report covers progress in FY 1995 on the following tasks: (1) Tests are ongoing to evaluate and compare the reactivity of fully radioactive glasses with that of glasses having the same compositions except for the absence of radionuclides under conditions representative of a high-level waste repository environment. Data from these tests will be used to evaluate the effect of radionuclides on the glass corrosion behavior and to determine the disposition of the radionuclides as the glass corrodes. Static dissolution tests and unsaturated tests are being conducted with several Defense Waste Processing Facility (DWPF) and West Valley Demonstration Project (WVDP) glasses. (2) A series of static dissolution tests is being performed to compare the corrosion behavior of nuclear waste glasses made with SRL 131 and SRL 202 frits at different S/V ratios. The S/V ratio affects the extent to which dissolved glass species are diluted; the solution chemistry then affects continued glass dissolution. The solutions generated in tests at high S/V ratios are conducive to the formation of alteration phases that may be deleterious to the glass. After long time periods, the glass dissolution rates of both glasses increase coincidentally with the formation of analcime and other alteration phases. However, the release of radionuclides from the glasses into solution is controlled by their individual solubilities.

    The Emergency Management Training Program Guide to Good Practice is a project of the Training Resources and Data Exchange (TRADE) Emergency Management Issues Special Interest Group (EMI SIG). EMI SIG members expressed interest in a resource to assist in development of a comprehensive emergency management training program. This publication provides guidelines, methods, and materials for EMI SIG members to use, assisting in complete and effective emergency management programs. The purposes of the Emergency Management Training Program Guide to Good Practice are: Provide guidance in the development and management of Emergency Management (EM) training programs; Assist EM trainers to incorporate components of the DOE Emergency Management System philosophy of planning, preparedness, readiness assurance, and response into EM training programs; Help EM training managers meet EM training requirements and conditions established by current regulations and policies; Supplement other TRADE EMI SIG documents and complement individual facility training documents. This program is designed for emergency management personnel who are responsible for providing or overseeing EM training but who do not necessarily possess expertise in developing training. It provides good practices from the manager's point of view on how to produce, administer, and document facility EM training programs in the spirit of the DOE EM system philosophy. Basic guidance is also included for personnel who design, develop, deliver, and/or evaluate EM training programs or parts. This guidance includes key points of EM training programs and identifies other documents that contain useful and/or more detailed training information.

  4. Laboratory and Field Studies Related to Radionuclide Migration at the Nevada Test Site in Support of the Underground Test Area and Hydrologic Resources Management Projects

    This report details the work of Chemistry Division personnel from Los Alamos National Laboratory in FY 2001 for the U. S. Department of Energy National Nuclear Security Administration Nevada Operations Office (NNSA/NV) under its Defense Programs and Environmental Restoration divisions. Los Alamos is one of a number of agencies collaborating in an effort to describe the present and future movement of radionuclides in the underground environment of the Nevada Test Site. This fiscal year we collected and analyzed water samples from a number of expended test locations at the Nevada Test Site. We give the results of these analyses and summarize the information gained over the quarter century that we have been studying several of these sites. We find that by far most of the radioactive residues from a nuclear test are contained in the melt glass in the cavity. Those radionuclides that are mobile in water can be transported if the groundwater is moving due to hydraulic or thermal gradients. The extent to which they move is a function of their chemical speciation, with neutral or anionic materials traveling freely relative to cationic materials that tend to sorb on rock surfaces. However, radionuclides sorbed on colloids may be transported if the colloids are moving. Local conditions strongly influence the distribution and movement of radionuclides, and we continue to study sites such as Almendro, which is thermally quite hot, and Nash and Bourbon, where radionuclides had not been measured for 8 years. We collected samples from three characterization wells in Frenchman Flat to obtain baseline radiochemistry data for each well, and we analyzed eight wells containing radioactivity for {sup 237}Np, using our highly sensitive ICP/MS. We have again used our field probe that allows us to measure important groundwater properties in situ. We conclude our report by noting document reviews and publications produced in support of this program.

    In 2006, Honeywell Federal Manufacturing & Technologies (FM&T) announced an updatedvision statement for the organization. The vision is “To be the most admired team within the NNSA [National Nuclear Security Administration] for our relentless drive to convert ideas into the highest quality products and services for National Security by applying the right technology, outstanding program management and best commercial practices.” The challenge to provide outstanding program management was taken up by the Program Management division and the Program Integration Office (PIO) of the company. This article describes how Honeywell developed and deployed a program management maturity model to drive toward excellence.

    Program fact sheet highlighting federal requirements for GHG emissions management, FEMP services to help agencies reduce emissions, and additional resources. The U.S. Department of Energy (DOE) Federal Energy Management Program (FEMP) assists Federal agencies with managing their greenhouse gas (GHG) emissions. GHG management entails measuring emissions and understanding their sources, setting a goal for reducing emissions, developing a plan to meet this goal, and implementing the plan to achieve reductions in emissions. FEMP provides the following services to help Federal agencies meet the requirements of inventorying and reducing their GHG emissions: (1) FEMP offers one-on-one technical assistance to help agencies understand and implement the Federal Greenhouse Gas Accounting and Reporting Guidance and fulfill their inventory reporting requirements. (2) FEMP provides training, tools, and resources on FedCenter to help agencies complete their annual inventories. (3) FEMP serves a leadership role in the interagency Federal Working Group on Greenhouse Gas Accounting and Reporting that develops recommendations to the Council on Environmental Quality (CEQ) for the Federal Greenhouse Gas Accounting and Reporting Guidance. (4) As the focus continues to shift from measuring emissions (completing inventories) to mitigating emissions (achieving reductions), FEMP is developing a strategic planning framework and resources for agencies to prioritize among a variety of options for mitigating their GHG emissions, so that they achieve their reduction goals in the most cost-effective manner. These resources will help agencies analyze their high-quality inventories to make strategic decisions about where to use limited resources to have the greatest impact on reducing emissions. Greenhouse gases trap heat in the lower atmosphere, warming the earth's surface temperature in a natural process known as the 'greenhouse effect.'     The Nuclear Waste Management Program Summary Document outlines the operational and research and development (R and D) activities of the Office of Nuclear Waste Management (NEW) under the Assistant Secretary for Nuclear Energy, US Department of Energy (DOE). This document focuses on the current and planned activities in waste management for FY 1981. This Program Summary Document (PSD) was prepared in order to explain the Federal nuclear waste management and spent fuel storage programs to Congress and its committees and to interested members of the public, the private sector, and the research community. The national energy policy as it applies to waste management and spent fuel storage is presented first. The program strategy, structure, budget, management approach, and public participation programs are then identified. The next section describes program activities and outlines their status. Finally, the applicability of departmental policies to NEW programs is summarized, including field and regional activities, commercialization plans, and environmental and socioeconomic implications of waste management activities, and international programs. This Nuclear Waste Management Program Summary Document is meant to serve as a guide to the progress of R and D and other energy technology programs in radioactive waste management.
