1200 delegates from 175 member countries attended the 45th World Health Assembly in Geneva. Everyone at the Assembly ratified measures to prevent and control AIDS. 12 countries intended to do long term planning for community based care for AIDS patients. Further the Assembly denounced instances where countries and individuals denied the gravity of the AIDS pandemic. In fact, it expressed the importance for urgent and intensive action against HIV/AIDS. The assembly backed proposals to prevent and control sexually transmitted diseases that affect AIDS patients, especially hepatitis B. For example, in countries with hepatitis B prevalence 8% (many countries in Sub-Sahara Africa, Asia, the Pacific region, and South America), health officials should introduce hepatitis B vaccine into their existing immunization programs by 1995. By 1997, this vaccine should be part of all immunization programs. The Assembly was aware of the obstacles of establishing reliable cold chains for nationwide distribution, however. Delegates in Committee A objected to the fact that 50% of the populations of developing countries continued to have limited access to essential drugs. They also expressed disapproval in implementation of WHO's 1988 ethical criteria for promotion of drugs which WHO entrusted to the Council for International Organisations of Medical Sciences (CIOMS). CIOMS lacked WHO's status and thus could not effectively monitor drug advertising. In fact, the pharmaceutical industry as well as WHO provided the funds for a meeting of 25 experts to discuss principles included in the ethical criteria. At least 4 countries insisted that WHO have the ultimate authority in monitoring drug advertising. Delegates did adopt a compromise resolution on this topic which required that industry promotion methods be reported to the 1994 Assembly via the Executive Board. The Assembly requested WHO to establish an international advisory committee on nursing and midwifery and to improve the network of

Delegation, an important concept for nursing students to learn and practice, is central to registered nurse (RN) performance, and important on the NCLEX-RN examination. Nursing faculty members from an ADN program designed a descriptive study to evaluate planned versus actual delegation in the curriculum, and a second study to evaluate an intervention on delegation. Study One assessed the presence of delegation in each nursing course. Statistical analysis compared the planned implementation with the results for student definitions of delegation, and identification of the five rights of delegation based on the National Council of State Boards of Nursing (NCSBN) definition and five rights. Study one results are shared. Study Two utilized a comparison of pre-to-post intervention measures. Students were asked to complete eight steps of a delegation exercise and determine what could be delegated to an unlicensed assistant, and what should be completed by the RN. Answers were coded and entered into SPSS. Statistical analysis compared each student's ability to correctly identify the five rights of delegation prior to the exercise, against the ability to correctly answer five questions two weeks post exercise. Significant improvement (p< 0.05) occurred on each measure. Recommendations are discussed.

Ninety-seven percent of middle level managers feel they face work overload. The source of this problem may lie in a failure to delegate tasks effectively. If a manager does not effectively delegate, he/she is more likely to report fatigue, stress and depression. Conversely, effectively delegating tasks will improve overall job quality, professional growth in subordinates and cooperation within the team. While it is necessary for managers to delegate tasks effectively, few can be considered to be good natural 'delegators'. Learning and training is necessary for managers to develop the necessary confidence and maturity. This article describes the definitions, purpose and benefits of delegation; factors related to effective delegation; strategies related to successful delegation and skills related to performing cross-cultural delegations. Such issues have seldom been addressed in previous articles. Moreover, nursing implications and suggested applications in nursing practice, education, management and research are mentioned. This article is intended to provide directions to nursing managers and subordinates to promote delegation knowledge and skills.

In France, as in most industrialized countries, health expenditures are predominantly financed by public money (taxes and contributions from employees and employers) and their share in the Gross Domestic Product is growing steadily for decades. The weight of the health sector upon the national economy is already extremely heavy. Does wisdom lie in launching action programmes aimed at uncertain returns? No doubt that the issue of task delegation is a painful dilemma to health workforce strategists.

Social participation and attention to the actions of government have increased dramatically in Ecuador. It is crucial that political debate be broadened concerning the functioning of the National Assembly, thereby opening greater public opportunities for participation. All social groups should be guaranteed access to the debate; expansion of the public sphere is essential for development of effective mechanisms of social inclusion. Those with no capacity to defend their own interests must have a voice. The National Assembly, in addition to reforming the Constitution, must reinforce the role of public men and statesmen at all levels of government. Statesmen place the common interest over special interests and create coalitions to effect necessary changes. The National Assembly must reorient the emphasis of government activities to give all sectors equal opportunity and access to basic public services. The role of the government must be redefined, which includes being equipped with better tools for management and control and with mechanisms for accountability at a time when many believe that globalization and market forces by themselves should dictate the rhythms of political, economic, and social life. Diversity should be respected. Nongovernmental organizations can be of great assistance in fostering dialogue, cooperation, solidarity, and consensus. Ecuadorians must support the goal of human and sustainable development.

Often, individuals take personal delegation skills for granted and assume the presence of expertise with the practice of delegation, which may not be the case. Those assumptions can be found at both ends of the process, with the manager and the employee. Every time a manager places an employee in a job and gives him or her a job description or a set of instructions, the manager has delegated. The manager has placed someone in a position to perform operations for which ultimately the manager is responsible. Delegation is both a process and a condition. The process is the act of assigning work to an employee; the condition of delegating a job is a thorough and mutual understanding between the supervisor and the employee of specific results and methods by which these results can be achieved. The condition goes far beyond the simple process of assigning a job. The point at which many managers fail in delegating is in neglecting to move past the process and take the required steps to establish a true condition of delegation. Failure to delegate is the leading cause of managers retarding their professional growth. In the case of a workaholic--someone who fails to learn the value of delegation--the job soon becomes too much, and the effectiveness of the department may suffer. By reducing the burden of technical duties and busy work, managers will find that it is possible to be more effective and actually spend more time managing. A number of the reasons why managers fail to delegate are complex and subconscious, such as insecurity, fear of competition and even fear of not being recognized for accomplishments achieved. Other reasons for failing to delegate are habit and shortages of staff members or time. Delegation is an investment in time. The eventual gain from such an investment, which may temporarily cause the department to fall further behind during a training period, outweighs the costs. Survey of Careers and Achievements on Delegates in JSRT International Delegation Projects.

Japanese society of radiological technology (JSRT) categorizes three international delegation projects; short-term studying abroad program (STSAP), international academic society visit program (overseas) (IASVP), and international internship visit program (Stanford University) (IIVP) for driving globalization of JSRT. In this survey, we conducted a questionnaire evaluating effectiveness of the international delegations. The survey covered 50 delegates of STSAP, 180 delegates of IASVP, and 100 delegates of IIVP. This survey includes detailed histories of career, current position, academic articles, and presentations as a first presenter before and on, and after each program. We categorized into six groups (change career, promoted in a position in hospital, kept a current position in hospital, promoted in a position in university, kept a current position in university, and others) in three programs. The response rate is approximately 58% (191/330 delegators). In all programs, almost all the delegates were radiological technologists in the hospital. They had reported a lot of academic articles and made a lot of presentations, and promoted in the hospital and/or university. STSAP, IASVP and IIVP were descending order of the average number of the articles as a first author and presentations as a first presenter. They published more the academic articles in Japanese than in English compared to JJRT and RPT. Therefore, research achievements and human resource conducted by this project provide great technologists and technique, and education. For further JRST globalization, it is desirable that we can continue these international delegations and verify the effectiveness.

Registered nurses can no longer expect to perform all patient care activities in this age of decentralization, shared governance, and redefining of work duties to include nurse extenders. Delegation is a much needed skill for hospital nurses, because it can free them to deal with professional activities of care, if it is learned and practiced over time. Practitioners of delegation must learn how to assess readiness for delegation in the extender and must recognize that delegation should be a planned process, not an act of desperation. Clear communication is critical at all stages of the process, and evaluation is necessary to be sure that the delegated tasks were completed accurately and promptly. Delegated separation is the transfer of responsibility for maintaining separation between aircraft or vehicles from air navigation service providers to the relevant pilot or flight operator. The effects of delegated separation and traffic display information level were collected through performance, workload, and situation awareness measures. The results of this study show benefits related to the use of conflict detection alerts being shown on the UAS operator's cockpit situation display (CSD), and to the use of full delegation. Overall, changing the level of separation responsibility and adding conflict detection alerts on the CSD was not found to have an adverse effect on performance as shown by the low amounts of losses of separation. The use of conflict detection alerts on the CSD and full delegation responsibilities given to the UAS operator were found to create significantly reduced workload, significantly increased situation awareness and significantly easier communications between the UAS operator and air traffic controller without significantly increasing the amount of losses of separation.

Doctors and nurses who had completed their training since 1960 were more likely to favour delegation than those who had completed their training before 1960. This suggests that delegation may become more common. However, the finding that there is considerable opposition to delegation and that this opposition is often based on feelings of professional threat suggests that many doctors may not be ready to experiment with ways of expanding the nurse's role in general practice. The credential delegation protocol allows a client program to delegate a user's credentials to a service such that that service may make requests of other services in the name of that user. The protocol defines a REST service that works alongside other IVO services to enable such a delegation in a secure manner. In addition to defining the specifics of the service protocol, this document describes how a delegation service is registered in an IVOA registry along with the services it supports. The specification also explains how one can determine from a service registration that it requires the use of a supporting delegation service.

To describe how registered nurses (RNs) perceive delegation to unlicensed personnel (UP) in a municipal healthcare context in Sweden. Within municipal health care RNs often delegate tasks to UP. The latter have practical training, but lack formal competence. Twelve RNs were interviewed and the material was analysed using a phenomenographic approach. Owing to a shortage of RNs, delegation is seen as a prerequisite for a functioning organization. This necessity also involves a number of perceived contradictions in three areas: (1) the work situation of RNs - facilitation and relief vs. lack of control, powerlessness, vagueness regarding responsibility, and resignation; (2) the relationship with unlicensed personnel - stimulation, possibility for mentoring, use of UP competence and the creation of fairness vs. questioning UP competence; and (3) The patients - increase in continuity, quicker treatment, and increased security vs. insecurity (with respect to, for example, the handling of medicine). Registered nurses perceptions of delegation within municipal healthcare involve their own work situation, the UP and the patients. Registered nurses who delegate to UP must be given time for mentoring such that the nursing care is safe care of high quality. © 2011 The Authors. In many Western countries, options for citizens to influence public services are increased to improve the quality of services and democratize decision making. Possibilities to influence are often cast into Albert Hirschman's taxonomy of exit (choice), voice, and loyalty. In this article we identify delegation as an important addition to this framework. Delegation gives individuals the chance to practice exit/choice or voice without all the hard work that is usually involved in these options. Empirical research shows that not many people use their individual options of exit and voice, which could lead to inequality between users and nonusers. We identify delegation as a possible solution to this problem, using Dutch health care as a case study to explore this option. Notwithstanding various advantages, we show that voice and choice by delegation also entail problems of inequality and representativeness.

Delegation is an essential process that allows nurses to function more effectively and efficiently. The Association of Rehabilitation Nurses' (ARN) Southeast Texas Chapter research committee developed a survey to study registered nurses (RN) practices and knowledge of delegation to unlicensed assistive personnel. State boards of nursing determine delegation practices, so the survey was sent only to Texas ARN members. Benners' Novice to Expert theory was used to study delegation practices based on years of experience, certification, and education. Survey Monkey was used with a questionnaire developed by the research committee. Descriptive statistics analyzed data from the survey's 73 respondents, and chi-square measured significance of differences based on years of experience and certification (yes or no). Data show that delegation knowledge does not necessarily translate to practice, especially when looking at specific tasks performed by certified rehabilitation registered nurses (CRRNs) and non-CRRNs. The data support continued study of this important issue; 93.7% of respondents say delegation requires further discussion.

An Extended Role-Based Access Control Model for Delegating Obligations

The main aim of access control models is to provide means to simplify the management of the security policy, which is a fastidious and error-prone task. Supporting delegation is considered as an important mean to decentralize the administration and therefore to allow security policy to be more flexible and easier to manipulate. Our main contribution is the proposition of a unified model to the administration and delegation of obligations. Managing such delegations implies more requirements than managing traditional privileges delegation. In fact, delegating obligations may include two interpretations: the delegation of the obligation and the delegation of the responsibility related to this obligation. Therefore, it is important to deal with these two notions separately. Moreover, since delegating an obligation involves the delegation of sanctions, then the consent of the user who receives this delegation may be required in some cases. We address in this paper these requirements and we propose a formalism to deal with them.

  Leadership and confidence in delegation are two important explanatory constructs of nursing practice. The relationship between these constructs, however, is not clearly understood. To be successful in their roles as leaders, regardless of their experience, registered nurses (RNs) need to understand how to best delegate. The present study explored and described the relationship between RN leadership styles, demographic variables and confidence in delegation in a community teaching hospital. Utilizing a cross-sectional survey design, RNs employed in one acute care hospital completed questionnaires that measured leadership style [Path-Goal Leadership Questionnaire (PGLQ)] and confidence in delegating patient care tasks [Confidence and Intent to Delegate Scale (CIDS)]. Contrary to expectations, the data did not confirm a relationship between confidence in delegating tasks to unlicensed assistive personnel (UAPs) and leadership style. Nurses who were diploma or associate degree prepared were initially less confident in delegating tasks to UAPs as compared with RNs holding a bachelor's degree or higher. Further, after 5 years of clinical nursing experience, nurses with less educational experience reported more confidence in delegating tasks as compared with RNs with more educational experience. The lack of a relationship between leadership style and confidence in delegating patient care tasks were discussed in terms of the PGLQ classification criteria and hospital unit differences. As suggested by the significant two-way interaction between educational preparation and clinical nursing experience, changes in the nurse's confidence in delegating patient care tasks to UAPs was a dynamic changing variable that resulted from the interplay between amount of educational preparation and years of clinical nursing experience in this population of nurses. Human kin cooperation is universal, leading researchers to label humans as “cooperative breeders.” Despite widespread interest in human cooperation, there has been no systematic study of how household economic decision making occurs. The aging of society leads on the one hand to increasing case numbers and on the other hand to a reduction in the number of physicians available for patient treatment. The delegation and substitution of medical duties as a tried and tested method is increasingly being recommended in order to compensate for the lack of physicians. The Berufsverband der Deutschen Urologen (BDU, Professional Association of German Urologists) supports the guiding principle of the Bundesärztekammer (Federal Medical Council) of "delegation yes, substitution no" and rejects a substitution of medical duties by non-medical academic health personnel. Against the background of the demographic changes, the increasing need for treatment and the current deficiency of junior physicians, a more extensive inclusion of well-qualified and experienced non-medical personnel by the delegation of medically responsible duties (medical scope of practice) can be an appropriate measure to maintain a good medical service in practices, hospitals and nursing homes.

Maturing technologies and complex payloads coupled with a future objective to reduce the logistics burden of current unmanned aerial systems (UAS) operations require a change to the 2-crew employment paradigm. Increased automation and operator supervisory control of unmanned systems have been advocated to meet the objective of reducing the crew requirements, while managing future technologies. Specifically, a delegation control employment strategy has resulted in reduced workload and higher situation awareness for single operators controlling multiple unmanned systems in empirical studies1,2. Delegation control is characterized by the ability for an operator to call a single "play" that initiates prescribed default actions for each vehicle and associated sensor related to a common mission goal. Based upon the effectiveness of delegation control in simulation, the U.S. Army Aeroflightdynamics Directorate (AFDD) developed a Delegation Control (DelCon) operator interface with voice recognition implementation for play selection, real-time play modification, and play status with automation transparency to enable single operator control of multiple unmanned systems in flight. AFDD successfully demonstrated delegation control in a Troops-in-Contact mission scenario at Ft. Ord in 2009. This summary showcases the effort as a beneficial advance in single operator control of multiple UAS.

This study is a qualitative, descriptive study of how registered nurses (RNs) (N=33) in leadership roles in institutionalized long-term care settings delegate care, including their strategies and processes for delegating care, and their perceptions of barriers to effective delegation and potential benefits of delegation. Findings indicate two key approaches to delegation, including the "follow the job description" approach, emphasizing adherence to facility-level roles and job descriptions, and the "consider the scope of practice" approach, emphasizing consideration of multiple aspects of scope of practice and licensure, and the context of care. While the former resulted in more clarity and certainty for the RN, the latter facilitated a focus on quality of resident care outcomes as linked to the delegation process. Barriers to effective delegation were comparable among RNs using either approach to delegation, and almost all RNs could describe benefits of delegation for long-term care.

Delegation practices between registered nurses and nursing assistive personnel.

To understand registered nurses' (RNs) and nursing assistive personnel's (NAP) perceptions of delegation practices in delivery of oncology patient care. No research to date describes how RNs and NAP communicate and interact during the delegation process. An understanding of the nature of communication during delegation offers direction for how RNs and NAP can improve collaboration. Qualitative descriptive study. Participants described conflict as a central theme during delegation. Sources of conflict varied between RNs and NAP. Successful delegation is characterised by effective communication, teamwork and initiative. Successful delegation depends on the quality of RN and NAP working relationships, timely ongoing communication, initiative and a willingness to collaborate. Nurse managers play a key role in the facilitation of delegation practices. Developing clear guidelines for RN and NAP patient reporting and providing opportunities to discuss conflict-related issues is essential. RNs would benefit from acquiring competency in how to conduct reports, resolve conflicts, and how to convey their role in patient care management. Nursing assistive personnel would benefit from developing competency in using effective communication skills for giving feedback, clarifying tasks and patient status and resolving conflict.

Task Delegation and Burnout Trade-offs Among Primary Care Providers and Nurses in Veterans Affairs Patient Aligned Care Teams (VA PACTs).

Appropriate delegation of clinical tasks from primary care providers (PCPs) to other team members may reduce employee burnout in primary care. However, (1) the extent to which delegation occurs within multidisciplinary teams, (2) factors associated with greater delegation, and (3) whether delegation is associated with burnout are all unknown. We performed a national cross-sectional survey of Veterans Affairs (VA) PCP-nurse dyads in Department of VA primary care clinics, 4 years into the VA's patient-centered medical home initiative. PCPs reported the extent to which they relied on other team members to complete 15 common primary care tasks; paired nurses reported how much they were relied on to complete the same tasks. A composite score of task delegation/reliance was developed by taking the average of the responses to the 15 questions. We performed multivariable regression to explore predictors of task delegation and burnout. Among 777 PCP-nurse dyads, PCPs reported delegating tasks less than nurses reported being relied on (PCP mean ± standard deviation composite delegation score, 2.97± 0.64 [range, 1-4]; nurse composite reliance score, 3.26 ± 0.50 [range, 1-4]). Approximately 48% of PCPs and 35% of nurses reported burnout. PCPs who reported more task delegation reported less burnout (odds ratio [OR], 0.62 per unit of delegation; 95% confidence interval [CI], 0.49-0.78), whereas nurses who reported being relied on more reported more burnout (OR, 1.83 per unit of reliance; 95% CI, 1.33-2.5). Task delegation was associated with less burnout for PCPs, whereas task reliance was associated with greater burnout for nurses. The aim of this study was to understand how nurses use critical thinking to delegate nursing care. Nurses must synthesize large amounts of information and think through complex and often emergent clinical situations when making critical decisions about patient care, including delegation. A qualitative, descriptive study was used in this article. Before delegating, nurses reported considering patient condition, competency, experience, and workload of unlicensed assistive personnel (UAP). Nurses expected UAP to report significant findings and have higher level knowledge, including assessment and prioritizing skills. Successful delegation was dependent on the relationship between the RN and the UAP, communication, system support, and nursing leadership. Nurses reported frequent instances of missed or omitted routine care. Findings from this project provide insight into factors that influence delegation effectiveness. These can guide CNOs and frontline nurse leaders to focus on implementing strategies to mitigate the consequence of missed care. Ineffective delegation of basic nursing care can result in poor patient outcomes, potentially impacting quality measures, satisfaction, and reimbursement for the institution.

Acute pain management is an interprofessional and interdisciplinary task and requires a good and trustful cooperation between stakeholders. Despite provisions in Germany according to which medical treatment can only be rendered by a formally qualified physician ("Arztvorbehalt"), a physician does not have to carry out every medical activity in person. Under certain conditions, some medical activities can be delegated to medical auxiliary personnel but they need to be (1) instructed, (2) supervised and (3) checked by the physician himself; however, medical history, diagnostic assessment and evaluation, indications, therapy planning (e.g. selection, dosage), therapeutic decisions (e. g. modification or termination of therapy) and obtaining informed consent cannot be delegated. With respect to drug therapy, monitoring of the therapy remains the personal responsibility of the physician, while the actual application of medication can be delegated. From a legal perspective, the current practice needs to be stressed about what is within the mandatory requirements and what is not when medical activities are delegated to non-medical staff. The use of standards of care improves treatment quality but like any medical treatment it must be based on the physician's individual assessment and indications for each patient and requires personal contact between physician and patient. Delegation on the ward and in acute pain therapy requires the authorization of the delegator to give instructions in the respective setting. The transfer of non-delegable duties to non-medical personnel is regarded as medical malpractice.

This study uses a task delegation questionnaire to compare 1973 physician extender practices in seven primary care-oriented sites with a physician attitude survey made in 1969. One additional site using no physician extenders was included as a control. The study involves both major types of physician extenders (physician assistants and nurse practitioners) in ambulatory practices with at least one year of experience in using such personnel. Here, fairness is prediction accuracy to sum of all voters preferences for choices. In simulation, each voter selects choices arranged on one dimensional preference axis for voting. Acquaintance relationships among voters were generated as a random network, and each voter delegates some of his acquaintances who has similar preferences. We obtained simulation results from various acquaintance networks, and then averaged these results. Firstly, if each voter has enough acquaintances in average, proposed method can help predicting sum of all voters' preferences of choices from small number of vote. Secondly, if the number of each voter's acquaintances increases corresponding to an increase in the number of voters, prediction accuracy (fairness) from small number of vote can be kept in appropriate level.

Delegation--or empowerment--represents the essence of the supervisory task: getting things done through people. The terms are no different from each other; empowerment is simply delegation done properly. The process still fails for the same old reasons, and failure still causes the same kinds of problems. Delegation or empowerment involves authority; it is authority that is delegated, not responsibility, as commonly claimed. Under either name it is an imperfect process requiring subjective judgments and chronic risk. Although either label is acceptable--the few differences between delegation and empowerment are semantic only--the significant constant that must be present is a sense of task ownership on the part of the empowered employee.

Effective delegation improves job satisfaction, responsibility, productivity and development. The ageing population demands more nurses in long-term-care hospitals. Delegation and leadership promote cooperation among nursing staff. However, little research describes nursing delegation and leadership style. We investigated the relationship between registered nurses' delegation confidence and leadership in Korean long-term-care hospitals. Our descriptive correlational design sampled 199 registered nurses from 13 long-term-care hospitals in Korea. Instruments were the Confidence and Intent to Delegate Scale and Multifactor Leadership Questionnaire. Confidence in delegation significantly aligned with current-unit clinical experience, length of total clinical-nursing experience, delegation-training experience and leadership. Transformational leadership was the most statistically significant factor influencing delegation confidence. When effective delegation integrates with efficient leadership, staff can deliver optimal care to long-term-care patients. © 2016 John Wiley & Sons Ltd.

The EPSRC first principle of robotics, "robots should not be designed solely or primarily to kill or harm humans, except in the interests of national security", is challenged in detail here. Focusing on security and armed conflict, arguments are marshalled against the principle on ethical, legal, technical and security grounds. A new principle is proposed that robots should never be delegated with the decision to apply violent force to humans.

As health care institutions in the United States respond to shrinking budgets and nursing shortages by increasing the use of unlicensed assistive personnel (UAP), school nursing practice is changing from providing direct care to supervising activities delegated to UAP. Therefore, delegation is a critical area of concern for school nurses. The purpose of this qualitative research study was to explore values guiding the delegation of health care tasks to UAP in school settings from the perspective of the school nurse. An inquiry focus group was conducted with 64 Florida school nurses. Values guiding delegation were comprehensive knowledge, trust, and empowerment. These values provided a framework for guiding the delegation process.

School nurses are in a unique position to provide care for students with special health care needs in the school setting. The incidence of chronic conditions and improved technology necessitate care of complex health care needs that had formerly been managed in inpatient settings. Delegation is a tool that may be used by registered nurses to allow unlicensed assistive personnel to perform appropriate nursing tasks and activities while keeping in mind that the registered nurse ultimately retains accountability for the delegation. The legal parameters for nursing delegation are defined by State Nurse Practice Acts, State Board of Nursing guidelines, and Nursing Administrative Rules/Regulations. Delegation becomes more challenging when carried out in a non-health care setting. School administrators may not be aware of legal issues related to delegation of nursing care in the school setting. It is crucial for school nurses to have a working knowledge of the delegation process. Development of a specific delegation policy will ensure that delegation is carried out in a manner providing for safe and appropriate care in the school setting.
