    La fistule artério-veineuse native est l'accès vasculaire de choix pour l'hémodialyse chronique en raison de sa longévité, son taux faible de complication et de mortalité par rapport aux pontages artério-veineux et aux cathéters. Cependant, il arrive assez souvent que l'on assiste à des complications qui sont dominées par la sténose et la thrombose. C'est une étude rétrospective des complications ayants survenues pour 31 fistules artério-veineuses pour hémodialyse chronique des 200 fistules réalisées chez 200 patients au sein du service de chirurgie vasculaire du CHU Hassan II de Fès sur une période de trois ans, étendue de Janvier 2007 à Décembre 2009. Ces complications ont été présentés par les thromboses dans 14 cas soit 45,15% de l'ensemble des complications, les sténoses dans 4 cas (12,90%,) les anévrismes dans 4 cas (12,90%), les complications ischémiques dans 3 cas (9,67%), l'infection dans 3 cas (9,67%), l'hémorragie dans 2 cas (6,45%) et l’ hyperdébit dans un seul cas soit 3,22%. On a pu conserver 22 fistules soit 70,96% par traitement chirurgical ou endovasculaire, on a confectionné une nouvelle fistule dans 8 cas soit 25,80%, et on a adressé une patiente (3,22%) pour pose d'un cathéter veineux tunnelisé permanent. Les complications des fistules artério-veineuses pour l'hémodialyse chronique sont la principale cause de morbidité chez les patients hémodialysés, il est donc important de s'impliquer lors de leur création, et de donner un maximum d'attention quand ils sont manipulés. Ceci suggère la mise en place d'un programme de surveillance de ces fistules en raison de l'impact des complications sur la morbi-mortalité du patient hémodialysé et sur le plan financier.     Les hypertensions artérielles (HTA) d'origine endocrine restent une cause rare d'HTA, sa prévalence globale n'excède pas 4% des hypertendus. L'intérêt de la recherche des HTA endocrines réside dans la gravité de certaines formes parfois mortelles et le caractère potentiellement curable et réversible de ces HTA. Le but du travail est de déterminer le profil clinique, para clinique, étiologique et thérapeutique des HTA secondaires d'origine endocrine chez les patients suivis au service d'endocrinologie au CHU Mohamed IV à Marrakech. Il s'agit d'une étude descriptive prospective s’étalant sur une période de 4 ans incluant 45 patients ayant une HTA endocrinienne. La moyenne d’âge est de 44,89 ans, avec une nette prédominance du sexe féminine (sexe ratio de 0,49). Les étiologies des HTA endocrines étaient dominées par le phéochromocytome (17 cas), l'hypercorticisme (11 cas) et l'acromégalie (8 cas). L'HTA était paroxystique dans 24,4%. Elle était d'emblée sevère classée grade 3 dans 40% des cas. L'HTA a été compliquée de cardiopathie dans 24% des cas et de néphropathie dans 20% des cas. Le traitement curatif a permis une guérison de l'HTA chez 60% (27 cas). Le diagnostic des HTA secondaires endocrines est parfois difficile du fait de l'absence de spécificité clinique.     Le cancer colorectal représente un problème majeur de santé publique. Son incidence annuelle mondiale est d’environ un million de cas et la mortalité annuelle est de plus de 500 000 cas par an. Le foie est l’organe le plus fréquemment touché par les métastases. Leur survenue est observée dans 40 à 60% des cas (contemporaines dans 25% des cas). Bien que la résection chirurgicale soit le seul traitement curatif, nombre de patients ne peuvent en bénéficier en raison de l’importante diffusion des métastases hépatiques. Chimiothérapies et biothérapies systémiques sont les options conventionnelles de la prise en charge des métastases hépatiques multiples. C’est dans ces situations cliniques que les thérapies intra-artérielles peuvent jouer un rôle important. Dans cet article, nous présenterons les différentes thérapies endovasculaires applicables aux métastases hépatiques de cancers colorectaux, avec leurs indications, résultats et complications éventuelles.     Summary Ce travail rétrospectif analyse les particularités épidémiologiques de 543 cas de brûlures d'enfants, représentant 45,7% des admissions de notre centre, en vue de déterminer les éléments pouvant contribuer à renforcer la prévention, qui reste le traitement de choix de cette pathologie. La moyenne d'âge est de 4,25 ans avec une prédilection pour la tranche d'âge d'un à cinq ans, avec 42,5% des cas. Une atteinte masculine est retrouvée dans 63,5% des cas. La brûlure survient à domicile dans 85,1% et accidentellement dans 95% des cas. Les brûlures thermiques représentent 96,5% des causes dominées par les liquides dans 69,3% des cas. La surface cutanée brûlée est ≥ 20% dans 52,3%. La brûlure intéresse essentiellement les membres supérieurs (79,1%). 56,8% des enfants sont transférés par d'autres hôpitaux et le délai de prise en charge hospitalière est supérieur à 6 heures dans 65,5%.   7. Massage, Music and Art Therapy in Hospice: Results of a National Survey

    Context Complementary and alternative medicine (CAM) provides clinical benefits to hospice patients, including decreased pain and improved quality of life. Yet little is known about the extent to which U.S. hospices employ CAM therapists. Objectives To report the most recent national data regarding the inclusion of art, massage, and music therapists on hospice interdisciplinary teams and how CAM therapist staffing varies by hospice characteristics. Methods A national cross-sectional survey of a random sample of hospices (n=591; 84% response rate) from September 2008 to November 2009. Results Twenty-nine percent of hospices (169 of 591) reported employing an art, massage, or music therapist. Of those hospices, 74% employed a massage therapist, 53% a music therapist, and 22% an art therapist, and 42% expected the therapist to attend interdisciplinary staff meetings, indicating a significant role for these therapists on the patient’s care team. In adjusted analyses, larger hospices compared with smaller hospices had significantly higher odds of employing a CAM therapist (adjusted odds ratio (AOR) = 6.38, 95% CI 3.40, 11.99) and forprofit hospices had lower odds of employing a CAM therapist compared with nonprofit hospices (AOR = 0.52, 95% CI 0.32, 0.85). Forty-four percent of hospices in the Mountain/Pacific region reported employing a CAM therapist versus 17% in the South Central region. Conclusion Less than one-third of U.S. hospices employ art, massage, or music therapists despite the benefits these services may provide to patients and families. A higher proportion of large hospices, nonprofit hospices and hospices in the Mountain/Pacific region employ CAM therapists, indicating differential access to these important services. PMID:25555445

  8. STEAM: A National Study of the Integration of the Arts Into STEM Instruction and its Impact on Student Achievement

    The purpose of this study is to examine the relationship between exposure to the arts and performance in Science, Technology, Engineering, and Math (STEM) subjects. STEAM, an integration of arts-based instruction into science and math related fields, is viewed as an alternative to traditional STEM academies. The literature briefly examines the current state of STEM programs and the deficiencies in graduate quality and quantity and the call from employers for a more innovative workforce. Advocates for STEAM argue for arts as a means to improve creativity, collaboration, risk-taking and exploration. Arguments against arts in STEM are grounded in political opinions concerning arts funding and logistical complications of implementing STEAM. However, some schools and STEM programs have embraced the STEAM premise and have begun to integrate arts into the traditional curriculum. The 2009 National Assessment of Educational Progress (NAEP) dataset was utilized to determine a correlation between the number of arts credits earned and mathematics/science achievement. Results from the NAEP dataset indicated a correlation between the amount of arts credits and increased achievement scores in science and math. The same correlation was found when controlling for demographic factors such as gender, race, and socio-economic status (SES). Overall, the arts' greatest impact was on students identified as "at-risk" or underrepresented in STEM fields. Controlling for these variable groups, one can note the quantifiable differences in scores. Overall, findings of the study provide empirical support for the addition of arts in STEM.

  9. Contaminants evaluation of Marais des Cygnes National Wildlife Refuge in Kansas and Missouri, USA

    At the new Marais des Cygnes National Wildlife Refuge in Linn County, Kansas, and Bates County, Missouri, USA, we evaluated long-lived contaminants before acquisition of the land for the refuge. We sampled sediments at 16 locations and fish at seven locations. The samples were analyzed for metals and for chlorinated hydrocarbon compounds. Selected sediment samples also were analyzed for aliphatic hydrocarbons. Arsenic concentrations in sediment samples from six locations were elevated compared to US norms, but arsenic was not detected in any fish composite. Mercury concentrations in largemouth bass from two locations were comparable to the 85th percentile concentrations in nationwide fish collections. Most sediment concentrations of other metals were unlikely to have detrimental effects on biota. No chlorinated hydrocarbons were detected in any sediment sample. Chlordane compound concentrations in fish composites from two sites at the eastern end of the sampling area were 0.127 and 0.228 μg/g wet weight, respectively, which are high enough to cause concern. Most aliphatic hydrocarbons detected were found at low concentrations and probably were natural in origin. We concluded that there are no serious contaminants concerns within the project area, but past use of arsenical pesticides may mean a legacy of elevated soil arsenic levels in parts of the area and some use of banned pesticides such as chlordane and DDT likely is still occurring near the refuge.

    Introduction L'objectif était de comparer la prévalence de l'hypertension artérielle chez les meuniers dans la ville de Lubumbashi qui sont exposés d'une manière permanente aux bruits des machines de transformation des céréales à la prévalence de l'hypertension artérielle chez un groupe des gardiens. Méthodes Quic'est une étude descriptive transversale qui a concerné 286 meuniers et 115 agents d'une entreprise de gardiennage. Apres un consentement éclairé, un auto questionnaire a été administré aux enquêtés et des mesures de poids, taille, et tension artérielle ont été prises. Résultats La prévalence de l'HTA chez les meuniers était 49.3% et celle des gardes était 20.9%, pour un ratio de prévalence de 2.4. significativement élevée par rapport aux contrôles (49.3% vs 20.9%) et le ratio de prévalence était de 2.4. Les facteurs de risque tels que: l’âge, l'indice de masse corporelle, l'ancienneté, la durée de travail ont été significativement élevés chez les meuniers par rapport aux gardiens. Conclusion Nous avons trouvé une prévalence élevée de l'HTA chez les meuniers. Nos résultats suggèrent que cette prévalence est probablement due à l'environnement du milieu de travail des meuniers (bruits, vibration des machines) et ce dernier constitue un facteur de risque de l'hypertension artérielle.     Ninety-eight fecal samples were collected from 74 free-living mountain gorillas (Gorilla gorilla beringei) from the Parc National des Volcans, Rwanda, between July 1995 and January 1997 and examined for parasites by Sheather's sugar and zinc sulfate flotation methods, trichrome staining, and larval cultures. All samples contained at least one parasite. Seventeen endoparasites were identified, including eight protozoa, seven nematodes, one cestode, and one trematode. Two species of arthropod mite were also recovered from the fecal samples. Parasites observed on fecal examinations included strongyle/trichostrongyle-type eggs (72/74) (representing Oesphagostomum sp., Trichostrongylus sp., Hyostrongylus spp., and possibly Murshidia sp.), Strongyloides sp. (1/74), Trichuris trichiura (2/74), Probstmayria sp. (7/74), Anoplocephala sp. (63/74), Entamoeba hartmanni cysts and trophozoites (19/70), Endolimax nana cysts (31/70), Iodamoeba buetschlii cysts (11/70), Endolimax nana or Iodamoeba buetschlii trophozoites (63/70). Entamoeba coli cysts and trophozoites (14/70), Entamoeba histolytica trophozoite (1/70), Chilomastix sp. cysts and trophozoites (31/70), and Giardia sp. cysts (2/70). In addition, one ascarid and one trematode egg were seen. There were no significant differences in the prevalence of parasites between males and females and between age groups: however, infants and juveniles appeared to have a lower prevalence of Anoplocephala gorillae, and the silverbacked males appeared to have a higher prevalence of Probstmayria sp. Parasite prevalence was consistent among the five social groups studied except Susa group had a significantly lower prevalence of Anoplocephala gorillae. Trichuris trichiura, Strongyloides sp., Chilomastix sp., and Endolimax nana were identified for the first time in this population, and it is possible that these parasites were of human origin.     Environmental Art works range in scope from major permanent interventions in the landscape to less intrusive, more ephemeral site-specific installations constructed of materials from the local environment. Despite this range of intervention, however, these works all share in a tradition of art making that situates the artwork in direct response to the surrounding landscape. Andy Goldsworthy and Richard Long, for example, both favour methods that combine elements of both sculpture and performance in the creation of non-permanent interventions in the landscape, and both rely upon photographic, text-based, or video documentation as the only lasting indication of the works' existence. Similarly, Earth Scientists are responsible for interventions in the landscape, both physical and conceptual. For example, in Earth science, the periods of the geologic timescale - Cambrian, Ordovician, Silurian, etc. - were established by 19th century pioneers of geology at a time when they were believed to represent natural chapters in Earth history. Since the mid-20th century, stratigraphers have attempted to resolve ambiguities in the original definitions by defining stratotypes: sections of continuously deposited strata where a single horizon is chosen as a boundary. One such international stratotype, marking the Cambrian-Ordovician boundary, is defined at Green Point in Gros Morne National Park, Newfoundland. Boundary|Time|Surface was an ephemeral sculptural installation work constructed in June 2014. The main installation work was a fence of 52 vertical driftwood poles, 2-3 m tall, positioned precisely along the boundary stratotype horizon at Green Point in Newfoundland. The fence extended across a 150 m wave-cut platform from sea cliffs to the low-water mark, separating Ordovician from Cambrian strata. The installation was constructed by hand (with volunteer assistance) on June 22, as the wave-cut platform was exposed by the falling tide. During the remainder of the tidal cycle

    Background Initiation of antiretroviral therapy (ART) in the advanced stages of HIV infection remains a major challenge in sub-Saharan Africa. This study was conducted to better understand barriers and enablers to timely ART initiation in Rwanda where ART coverage is high and national ART eligibility guidelines first expanded in 2007–2008. Methods Using data on 6326 patients (≥15 years) at five Rwandan clinics, we assessed trends and correlates of CD4+ cell count at ART initiation and the proportion initiating ART with advanced HIV disease (CD4+ <200 cells/µl or WHO stage IV). Results Out of 6326 patients, 4486 enrolling in HIV care initiated ART with median CD4+ cell count of 211 cells/µl [interquartile range: 131–300]. Median CD4+ cell counts at ART initiation increased from 183 cells/µl in 2007 to 293 cells/µl in 2011–2012, and the proportion with advanced HIV disease decreased from 66.2 to 29.4%. Factors associated with a higher odds of advanced HIV disease at ART initiation were male sex [adjusted odds ratios (AOR) = 1.7; 95% confidence interval (CI): 1.3–2.1] and older age (AOR46–55+ vs. <25 = 2.3; 95% CI: 1.2–4.3). Among those initiating ART more than 1 year after enrollment in care, those who had a gap in care of 12 or more months prior to ART initiation had higher odds of advanced HIV disease (AOR = 5.2; 95% CI: 1.2–21.1). Conclusion Marked improvements in the median CD4+ cell count at ART initiation and proportion initiating ART with advanced HIV disease were observed following the expansion of ART eligibility criteria in Rwanda. However, sex disparities in late treatment initiation persisted through 2011–2012, and appeared to be driven by later diagnosis and/or delayed linkage to care among men. PMID:25562492

    Over a decade ago the Department of Energy started the design, and construction of five Nanoscale Science Research Centers at different national laboratories with the objective to provide research opportunities in Nanoscience for the scientific community worldwide. The Center for Nanoscale Materials (CNM) at Argonne National Laboratory was constructed in 2006, and opened its doors to the user community in 2007. Currently the CNM hosts over 400 user proposals a year. There are six research groups at the CNM that do work in nanophotonics, electronic and magnetic materials and devices, nanobio interfaces, nanofabrication and devices, x-ray nanoscale microscopy and theory and modeling. I work in the Nanofabrication and Devices group and my research career has covered the use of x-rays, electrons and ions in the pursuit of making the smaller and smaller structures and devices. At the CNM I have been able to push the limits of electron beam lithography, and expand the use of ion beams to large area nanofabrication. Some of our accomplishments include determining liquid-polymer interactions as a function of temperature, redefining proximity effect correction at the nanoscale (NanoPEC), measuring to less than 0.5% error the backscatter range for 100 KV electron beams and finding that the range is a function of the density of the substrate, fabrication of plasmonic slit waveguides, and using ions to create complex three dimensional structures for use in fluidics. None of these accomplishments are possible without detailed understanding of the physics and chemistry mechanisms involved during fabrication. This requires extensive theory and simulation work to validate our experimental results. The fruit of our work then is a full understanding of ``why'' we use certain processes for nanofabrication and not just a simple set of process recipes.     This paper provides the official recommendation of the Union Internationale des Associations d'Alpinisme (UIAA) Medical Commission to manage the problem of safe drinking water. The recommendation was accepted and authorized for publication by the Medical Commission during their annual meeting at Treplice, Tzechia, 2008. Safe water is essential for mountaineers worldwide in order to balance challenges associated with high altitude dehydration. The paper summarizes the advantages and disadvantages of several procedures used to procure safe drinking water in the mountains or at high altitude. Limitations or critical details, which may cause failure of the methods are mentioned systematically. We differentiate between "conventional" methods, which should be preferred because they produce safe water and "improvisation". The latter does not produce safe water but may be used if conventional methods are not available for any reason. They decrease the concentration of pathogenic microorganisms and by this they reduce the risk of enteral infection. Water filtration using a ceramic filter system or chemical disinfection is recommended as a standard method. Boiling water should be avoided because it is too fuel consuming and has the potential to increase deforestation. Generally, with regard to infections by water or food, all mountaineers should be vaccinated against hepatitis A and poliomyelitis in regions where they may be at-risk. PMID:19174294

    Introduction Le pied diabétique se définit comme l'ensemble des manifestations trophiques du pied survenant chez le diabétique par atteinte nerveuse, artérielle et ou infectieuse. Le pied diabétique est un problème majeur de santé publique à l’échelle mondiale avec un taux d'amputation de membres inférieurs très élevé. L’écho-doppler artériel des membres inférieurs est de nos jours incontournable dans la prise en charge du pied diabétique. Le but de cette étude est de montrer la place prépondérante qu'occupe l’écho-doppler artériel dans le bilan lésionnel du pied diabétique. Méthodes Nous avons mené une étude rétrospective monocentrique incluant 46 patients sur une période de 24 mois, de mars 2012 à mars 2014 à l'hôpital Saint-Jean de Dieu, un des deux hôpitaux de référence de la région de Thiès, doté depuis juillet 2011 d'un centre moderne de traitement du diabète et des maladies cardio-métaboliques (Diabcarmet). Dans les critères d'inclusion, nous avons sélectionné tous les patients diabétiques adressés pour un écho-doppler artériel des membres inférieurs dans le cadre d'une prise en charge du pied diabétique. Etaient exclus de l’étude, les patients artéritiques non-diabétiques et les patients diabétiques asymptomatiques référés pour un bilan écho-doppler de routine. Résultats Le sex-ratio était de 1.42 (27 hommes pour 19 femmes). L’âge moyen des patients était de 62,86 ans avec des extrêmes de 23 et 88 ans. 60% des patients (n=28) étaient âgés entre 50 et 70 ans. Le diabète de type 2 était retrouvé chez 95% des patients (n=44) alors que le diabète de type 1 représentait 5% (n=2). La moyenne d’évolution du diabète était estimée à 8 ans, avec des extrêmes de 2 et 20 ans.     The Bureau International des Poids et Mesures has initiated world-wide intercomparisons of pressure standards. The intercomparison in the regime of very low pressures, i.e. 10-4 to 1 Pa, using Ar as the test gas, has now been completed with the participation of laboratories in nine countries (in alphabetical order): Czechoslovakia, Federal Republic of Germany, France, India, Italy, Japan, People's Republic of China, UK and United States of America. In addition, five of these laboratories made measurements using H2 as the test gas. Four spinning-rotor gauges were employed as transfer standards. The achieved transfer uncertainty between the pilot laboratory (Physikalisch-Technische Bundesanstalt) and a participating laboratory lies between 0.16% and 0.30% (1σ) for Ar and has allowed an intercomparison at this level of accuracy. At all investigated pressures the standards in the various laboratories lie within an interval of +/-3% around the standard of the pilot laboratory. For a brief synopsis of the intercomparison results, the calibration data of each laboratory for one gas species are reduced to a single number, i.e., the average over the pressure range 10-3 to 1 Pa, although this procedure is somewhat arbitrary and obscures scatter and pressure-dependence of some calibration results. These averages deviate from their mean by not more than about 1%. Systematic deviations between some laboratories are clearly identifiable: however, for most of the laboratories it appears that these are compatible with the estimated uncertainties of the pressure generation and the transfer uncertainty.

    A landslide inventory is the most important information source for quantitative zoning of landslide susceptibility, hazard and risk. It should give insight into the location, date, type, size, activity and causal factors of landslides as well as resultant damage. In Europe, many countries have created or are creating national and/or regional landslide databases (LDBs). Yet little is known on their contents, completeness, format, structure, language use and accessibility, and hence on their ability to perform national or transnational landslide zoning. Therefore, this study presents a detailed analysis of existing national LDBs in the EU member states, EU official candidate and potential candidate countries, and EFTA countries, and their possible use for landslide zoning. These national LDBs were compared with a subset of 22 regional databases. Twenty-two out of 37 contacted European countries currently have national LDBs, and six other countries have only regional LDBs. In total, the national LDBs contain 633,696 landslides, of which 485,004 are located in Italy, while Austria, the Czech Republic, France, Norway, Poland, Slovakia, and the UK also have > 10,000 landslides in their LDBs. National LDBs are generally created in the official language of each country and 58% of them contain other natural hazards (e.g. floods and sinkholes). About 68% of the LDBs contain less than 50% of all landslides in each country, but a positive observation is that 60% of the LDBs are updated at least once a year or after a major event. Most landslide locations are collected with traditional methods such as field surveys, aerial photo interpretation and analysis of historical records. Currently, integration of landslide information from different national LDBs is hampered because of differences in language and classification systems for landslide type and activity. Other problems are that currently only half of the national LDBs have a direct link between spatial and alphanumeric

This review treats many aspects of salivary gland endoscopy, starting from anatomy to deal with the more advanced surgical techniques and analyses the main decisional algorithms proposed in the literature.     A new class of polyimides, synthesized by Langley Research Center, has been evaluated by the Getty Conservation Institute's Materials Science Group for possible art conservation applications. Polyimides are noted for resistance to high temperature, wear and radiation. They are thermally stable and soluble in some common solvents. After testing under simulated exposures for changes in color, permeability and flexibility, one coating, ODPA-3, 3-ODA may be used to protect bronze statues from corrosion. A test on stained glass windows was unsuccessful.

    Même après l'entrée en dialyse, la prévalence de l'hypertension artérielle (HTA) reste élevée. Elle est souvent greffée d'une forte morbi-mortalité, et altère la qualité de vie des patients hémodialysés chroniques. La « méthode du poids sec (PS) », établie par Scribner, présente un intérêt indéniable dans la gestion de l'HTA chez le patient hémodialysé. Le but de notre travail était de déterminer la prévalence et les facteurs de risque (FDR) de l'HTA chez nos hémodialysés chroniques et d'en tenter une réduction en nous basant sur une stratégie d’éducation thérapeutique basée sur les recommandations de Scribner. Nous avons mené une étude prospective interventionnelle en trois mois et en trois phases, auprès des 93 hémodialysés chroniques de l'Hôpital Al Farabi d'Oujda. En phase 1, nous avons déterminé la prévalence de l'HTA par la surveillance horaire de la tension artérielle (TA) durant la séance de dialyse pendant deux semaines, soit sur un total de 442 séances. Les patients ont également mesuré leur pression artérielle (PA) hors-centre de façon biquotidienne les jours de non-dialyse, soit un total de 1720 mesures hors-centre. L'HTA a été définie par une Pression Artérielle Systolique (PAS) supérieure ou égale à 140mmHg et/ou une Pression Artérielle Diastolique (PAD) supérieure ou égale à 90mmHg sur au moins 2 mesures. En phase 2, les patients hypertendus ont bénéficié d'une prise en charge basée sur les recommandations de Scribner et une éducation thérapeutique (ETP). En phase 3, nous évalué les retombées de notre prise en charge. Une HTA a été notée sur 231 séances chez 57 patients, soit une prévalence de 61,3%. La PAS moyenne était de 172,75±17,69 (145-220) mmHg. Les FDR retenus sont: l’âge, la PPID importante, le non respect des règles hygiéno-diététiques (RHD) et le rythme de 2 séances de dialyse/semaine.
