    The purpose of this program and set of procedures is to describe Mound`s plan to conduct and document Operational Readiness (OR) and Operational Readiness Reviews (ORR). The OR process provides a formal, auditable methodology for obtaining operational readiness of Category 1, Category 2, and Category 3 hazard facilities, systems, processes, or operations. This effort and subsequent review must, at a minimum, consider environmental, safety, health, technical, operational, programmatic and administrative activities. The ORR is used as a contractor line management tool for making initial start and restart decisions. It is the intent of this policy to establish the requirements for Mound OR and ORR projects.

    The purpose of this Standard is to establish a uniform process for when an ORR (1) of a new nuclear facility is to be started, (2) a previously operated facility is to be restarted, or (3) any new operating phase that results in a significant increase in existing hazards, or could introduce significant hazards not previously encountered is planned. This Standard describes a method to verify that the facility is physically ready to startup, that the managers and operators are prepared to manage and operate the facility in the proposed phase, and that the necessary infrastructure are in place. It also provides a methodology for determining the breadth and depth of the ORR so as to be consistent with the hazards and complexity of the proposed facility transition. This Standard provides procedures for the conduct of an ORR when required by the circumstances of the startup as specified in DOE 5480.31. The Standard also discusses principles and criteria for Readiness Assessments incident to the facility startup process when an ORR in not required.

    An Operational Readiness Review (ORR) at the Idaho National Engineering Laboratory`s (INEL`s) Waste Experimental Reduction Facility (WERF) was conducted by EG&G Idaho, Inc., to verify the readiness of WERF to resume operations following a shutdown and modification period of more than two years. It is the conclusion of the ORR Team that, pending satisfactory resolution of all pre-startup findings, WERF has achieved readiness to resume unrestricted operations within the approved safety basis. ORR appraisal forms are included in this report.

    The Sandia ACRR (a Hazard Category 2 Nuclear Reactor Facility) was defueled in June 1997 to modify the reactor core and control system to produce medical radioisotopes for the Department of Energy (DOE) Isotope Production Program. The DOE determined that an Operational Readiness Review (ORR) was required to confirm readiness to begin operations within the revised safety basis. This paper addresses the ORR Process, lessons learned from the Sandia and DOE ORRS of the ACRR, and the use of the ORR to confirm authorization basis implementation.

    In October 1989, a US shuttle lifted off from Cape Kennedy carrying the spacecraft Galileo on its mission to Jupiter. In November 1990, a second spacecraft, Ulysses, will be launched from Cape Kennedy with a mission to study the polar regions of the sun. The prime source of power for both spacecraft is a series of radioisotope thermoelectric generators (RTGs), which use plutonium oxide (plutonia) as a heat source. Several of the key components in this power system are required to ensure the safety of both the public and the environment and were manufactured at Oak Ridge National Laboratory (ORNL) in the 1980 to 1983 period. For these two missions, Martin Marietta Energy Systems, Inc. (Energy Systems), will provide an iridium-alloy component used to contain the plutonia heat source and a carbon-composite material that serves as a thermal insulator. ORNL alone will continue to fabricate the carbon-composite material. Because of the importance to DOE that Energy Systems deliver these high-quality components on time, performance of an Operational Readiness Review (ORR) of these manufacturing activities is necessary. Energy Systems Policy GP-24 entitled Operational Readiness Process'' describes the formal and comprehensive process by which appropriate Energy Systems activities are to be reviewed to ensure their readiness. This Energy System policy is aimed at reducing the risks associated with mission success and requires a management-approved readiness plan'' to be issued.   9. Operational Readiness Review Final Report For F-Canyon Restart. Phase 1

    An independent WSRC Operational Readiness Review was performed for the restart of Phase 1 processing in F-Canyon, Building 221-F. Readiness to restart the Second Plutonium Cycle process and solvent recovery was assessed. The ORR was conducted by an ORR board of ten members with the support of a subject matter expert. The chairman and four members were drawn from the Operational Safety Evaluation Department, ESH& QA Division; additional members were drawn from other WSRC divisions, independent of the F-Canyon operating division (NMPD). Based on the results of the readiness verification assessments performed according to the ORR plan and the validation of pre-restart corrective actions, the WSRC independent ORR Board has concluded that the facility has achieved the state of readiness committed to in the Restart Plan. Also, based on the scope of the ORR, it is the opinion of the board that F-Canyon Phase 1 processes can be restarted without undue risk to the safety of the public and onsite workers and without undue risk to the environment.

    This Implementation Plan (IP) has been prepared consistent with the requirements of U.S. Department of Energy (DOE) Order 425.1B, ''Startup and Restart of Nuclear Facilities'', and DOE-STD-3006-2000, ''Planning and Conduct of Operational Readiness Reviews'' (ORR) (DOE 2002). The scope of the DOE ORR is described in the RL ''Plan of Action, K Basin Sludge Water System'' (Veitenheimer 2003), prepared by DOE project line management and approved by the RL Manager, the designated Approval Authority, on March 20, 2003. The scope of the contractor ORR is described in the contractor ''Plan of Action for the K Basins Sludge Water System Operational Readiness Review'' (FH 2002a) which was prepared by Spent Nuclear Fuel (SNF) Project line management and approved by the DOE Richland Operations Office (RL) Manager on December 19, 2002. DOE Order 425.1B indicates that the Secretarial Officer is the Authorization Authority when substantial modifications are made to a Hazard Category 2 nuclear facility. This Authorization Authority has been delegated to the RL Manager by memorandum from Jessie Hill Roberson, dated February 5, 2003 (Roberson 2003). This IP provides the overall approach and guidelines for performance of the DOE ORR. Appendix A contains the Criteria and Review Approach Documents (CRAD), which define the review objectives and criteria as well as the approach for assessing each objective. ORR results will be published in a final report, as discussed in Section 9.4.

  12. Operational Readiness Review Final Report for Implementation of an Increased Facility Radioactive Source Inventory Limit & Shippingport Fuel 07/2002

    An Operational Readiness Review (ORR) was conducted by the US Department of Energy (DOE), Richland Operations Office (RL) to verify that an adequate state of readiness had been achieved for those activities associated with the removal of Shippingport fuel from the 271-T Facility (T Plant) as a Hazard Category 2 nuclear facility and the readiness of T Plant to operate within the revised safety basis (SB) inventory limits. The DOE ORR was conducted in two parts in accordance with the T Plant Operational Readiness Review DOE-RL Plan of Action. The DOE team observed the dry run of the Shippingport fuel removal process, in conjunction with the contractor ORR of June 2002. Autonomously, the DOE Team completed the ORR in July 2002 by determining the adequacy of the contractor's final ORR (CORR); conducting a sampling of T Plant procedures and processes to validate the CORR; and, observation of a dry run that demonstrated the ability of T Plant to seal a Shippingport Spent Fuel Canister. The DOE ORR Team has concluded that T Plant has demonstrated an appropriate state of readiness and recommends the RL Manager authorize Fluor Hanford to implement the revised safety basis (SB) inventory limits and begin Shippingport fuel removal operations as a Hazard Category 2 nuclear facility. The team further concludes that an adequate level of protection for the worker, public health and safety, and the environment is inherent in the T Plant operations. T Plant has demonstrated an enhanced safety culture. Although there is room for improvement in the T Plant operations as represented by the findings and observations annotated in the body of the report, the overall operation has demonstrated a high degree of professionalism and a discipline of operations that bodes well for the current efforts and future endeavors.

The manual also fails to detail the considerations involved in maintaining the state of readiness on a continuing basis. Both of the latter subjects are dealt with in some detail in the SSDC's Operational Readiness Workshop. The purpose of this document is to provide additional documentary material dealing with subjects introduced in SSDC-1, Occupancy-Use Readiness Manual, and SSDC-12, Safety Considerations in Evaluation of Maintenance Programs. In augmenting SSDC-1, Part 1 of this manual provides additional material related to process safety; in the case of SSDC-12, the subject of safety considerations in evaluation of maintenance programs is broadened in Part 2 to include maintenance of personnel systems and procedural systems as well as hardware.   15. Achieving TASAR Operational Readiness

    NASA has been developing and testing the Traffic Aware Strategic Aircrew Requests (TASAR) concept for aircraft operations featuring a NASA-developed cockpit automation tool, the Traffic Aware Planner (TAP), which computes traffic/hazard-compatible route changes to improve flight efficiency. The TAP technology is anticipated to save fuel and flight time and thereby provide immediate and pervasive benefits to the aircraft operator, as well as improving flight schedule compliance, passenger comfort, and pilot and controller workload. Previous work has indicated the potential for significant benefits for TASAR-equipped aircraft, and a flight trial of the TAP software application in the National Airspace System has demonstrated its technical viability. This paper reviews previous and ongoing activities to prepare TASAR for operational use.

    The primary technical objective for the WERF Restart Project is to assess, upgrade where necessary, and implement management, documentation, safety, and operation control systems that enable the resumption and continued operation of waste treatment and storage operations in a manner that is compliant with all environment, safety, and quality requirements of the US Department of Energy and Federal and State regulatory agencies. Specific processes that will be resumed at WERF include compaction of low-level compatible waste; size reduction of LLW, metallic and wood waste; incineration of combustible LLW and MLLW; and solidification of low-level and mixed low-level incinerator bottom ash, baghouse fly ash, and compatible sludges and debris. WERF will also provide for the operation of the WWSB which includes storage of MLLW in accordance with Resource Conservation and Recovery Act requirements.

    National Ignition Facility experiments involve the use of a variety of materials that generate a number of radiological issues. Along with the use of tritium and depleted uranium, shots generating neutrons create prompt radiation fields as well as fission and activation products. In order to assure readiness for these hazards, a series of readiness reviews was conducted as the hazards were introduced. Each step was built upon the previous steps, as well as the basic infrastructure and operating capability of the laser facility. A detailed preparation plan for the introduction of these hazards was developed. This included ensuring required equipment was in place and ready, all plans and procedures were developed, and personnel were trained and qualified to perform work in the environment. The approach for preparing the facility for operations under the new set of conditions, the preparations for the readiness reviews, the review process, as well as the approach to initial operations are discussed.     Sandia has implemented a formal process to verify that new or modified facilities and operations are ready to safely operate. The readiness review process focuses on the status of management systems, personnel, and systems, structures, and components to do work safely. The scope and depth of the review are tailored to match the potential consequences and the likelihood that the consequences could occur. The precepts and methodology of the process are applicable to verifying the readiness of enterprise systems, and should comprise the final element in developing and implementing an enterprise system. This paper describes the readiness review process, the key elements for success, lessons learned from Sandia's readiness assessment process, and outlines how the process can be applied to enterprise systems. Specific topics addressed include selecting the criteria, approach, and lines of inquiry to be used for the review; selecting members for the review team; team leader responsibilities; reporting and closing deficiencies; and, responsibilities of the facility/project owner and management.

    The objective of this manuscript is to present a systematic review of biosurveillance models that operate on select agents and can forecast the occurrence of a disease event. We define a disease event to be a biological event with focus on the One Health paradigm. These events are characterized by evidence of infection and or disease condition. We reviewed models that attempted to predict a disease event, not merely its transmission dynamics and we considered models involving pathogens of concern as determined by the US National Select Agent Registry (as of June 2011). We searched commercial and government databases and harvested Google search results for eligible models, using terms and phrases provided by public health analysts relating to biosurveillance, remote sensing, risk assessments, spatial epidemiology, and ecological niche modeling. After removal of duplications and extraneous material, a core collection of 6,524 items was established, and these publications along with their abstracts are presented in a semantic wiki at http://BioCat.pnnl.gov. As a result, we systematically reviewed 44 papers, and the results are presented in this analysis. We identified 44 models, classified as one or more of the following: event prediction (4), spatial (26), ecological niche (28), diagnostic or clinical (6), spread or response (9), and reviews (3). The model parameters (e.g., etiology, climatic, spatial, cultural) and data sources (e.g., remote sensing, non-governmental organizations, expert opinion, epidemiological) were recorded and reviewed. A component of this review is the identification of verification and validation (V&V) methods applied to each model, if any V&V method was reported. All models were classified as either having undergone Some Verification or Validation method, or No Verification or Validation. We close by outlining an initial set of operational readiness level guidelines for disease prediction models based upon established Technology Readiness

    In preparation for forming low-enriched uranium carbide/oxide (LEUCO) fuel kernels for the Advanced Gas Reactor (AGR) fuel development and qualification program, Idaho National Laboratory conducted an operational readiness review of the Babcock & Wilcox Nuclear Operations Group – Lynchburg (B&W NOG-L) procedures, processes, and equipment from January 14 – January 16, 2015. The readiness review focused on requirements taken from the American Society Mechanical Engineers (ASME) Nuclear Quality Assurance Standard (NQA-1-2008, 1a-2009), a recent occurrence at the B&W NOG-L facility related to preparation of acid-deficient uranyl nitrate solution (ADUN), and a relook at concerns noted in a previous review. Topic areas open for the review were communicated to B&W NOG-L in advance of the on-site visit to facilitate the collection of objective evidences attesting to the state of readiness.

The purpose of this document is to provide additional documentary material dealing with subjects introduced in SSDC-1 Occupancy-Use Readiness Manual, and SSDC-12, Safety Considerations in Evaluation of Maintenance Programs. In augmenting SSDC-1, Part I of this manual provides additional material related to process safety; in the case of SSDC-12, the subject of safety considerations in evaluation of maintenance programs is broadened in Part II to include maintenance of personnel systems and procedural systems as well as hardware. ``Maintenance`` is related more directly to the concept of operational readiness and an alternative analytical tree is provided for hardware maintenance program evaluation.

    The Ballistic Missile Office (BMO) has developed and employed a procedure to insure the flight readiness of a Peacekeeper guidance system. The goals of this additional review procedure (over and above acceptance testing) are to minimize the risk of the guidance system failure and/or achieving substantially less than the designed accuracy. The roles, mission and interaction of teams of personnel from seven associate contractors are discussed using flow diagrams. The paper also includes methods used to identify and resolve areas of concern as well as examples of actual concerns discovered. The scope is discused, including both the various technical disciplines involved and the lowest level of build and test areas reviewed. Resulting build and test improvements are shown.

    Large capital building projects benefit from an operational readiness strategy prior to new facility occupancy. St. Joseph's Healthcare used a structured approach for their readiness planning that included individual work plan meetings, tools for ensuring integration across programs and services and process improvement support to ensure a smooth transition. Over 1100 staff were oriented using a Train-the-Trainer model. Significant effort was required to co-ordinate the customized training, which involved "staffing up" to ensure sufficient resources for backfill. Operational readiness planning places additional demands on managers, requiring support and assistance from dedicated resources both prior to occupancy and several months post-move. PMID:25906468

    INTRODUCTION: The objective of this manuscript is to present a systematic review of biosurveillance models that operate on select agents and can forecast the occurrence of a disease event. One of the primary goals of this research was to characterize the viability of biosurveillance models to provide operationally relevant information for decision makers to identify areas for future research. Two critical characteristics differentiate this work from other infectious disease modeling reviews. First, we reviewed models that attempted to predict the disease event, not merely its transmission dynamics. Second, we considered models involving pathogens of concern as determined by the US National Select Agent Registry (as of June 2011). Methods: We searched dozens of commercial and government databases and harvested Google search results for eligible models utilizing terms and phrases provided by public health analysts relating to biosurveillance, remote sensing, risk assessments, spatial epidemiology, and ecological niche-modeling, The publication date of search results returned are bound by the dates of coverage of each database and the date in which the search was performed, however all searching was completed by December 31, 2010. This returned 13,767 webpages and 12,152 citations. After de-duplication and removal of extraneous material, a core collection of 6,503 items was established and these publications along with their abstracts are presented in a semantic wiki at http://BioCat.pnnl.gov. Next, PNNL’s IN-SPIRE visual analytics software was used to cross-correlate these publications with the definition for a biosurveillance model resulting in the selection of 54 documents that matched the criteria resulting Ten of these documents, However, dealt purely with disease spread models, inactivation of bacteria, or the modeling of human immune system responses to pathogens rather than predicting disease events. The generation of simulated tracking data to stress the Flight Dynamics System and the Flight Dynamics Team was important for satisfying the testing goal of demonstrating that the software and the team were ready to fly the operational mission. The simulated tracking was generated in a manner to incorporate the effects of errors in the baseline dynamical model, errors in maneuver execution and phenomenology associated with various tracking system based components. The ability of the mission team to overcome these challenges in a realistic flight dynamics scenario indicated that the team and flight dynamics system were ready to fly the LADEE mission.     The final activity ushering in the new phase of the existence of Cluster was performed last week, with the successful deployment of the antenna and experiment booms, three altogether on each spacecraft. This was done through careful manoeuvring, under the control of the ESOC operations team in Darmstadt. Over the next few months all scientific instruments onboard the spacecraft will be gradually brought to life. The same instrument on each spacecraft will be switched on, one after another. Over a period of three months the instruments will undergo a series of health and calibration checks. By early December all 44 instruments on the four spacecraft will be operational and ready to start the scientific mission. The four Cluster spacecraft were launched in two pairs from the Baikonur cosmodrome in Kazakhstan - Salsa and Samba on 16 July and Rumba and Tango on 9 August. Their current highly elliptical orbits vary from 17.200 km at perigee to 120.600 km at apogee. By making simultaneous measurements in a tetrahedral formation, the Cluster quartet will be able to make the most detailed three-dimensional study yet of the Sun-Earth connection and of the changes and processes taking place in near-Earth space. For the first time ever in space history, four identical spacecraft are operated simultaneously in orbit, opening new horizons for future multi-spacecraft missions. The Cluster project involves more than 70 laboratories and over 250 scientists from many countries, including Europe, the United States, Russia, China, Japan, India, Israel, Hungary, Poland and the Czech Republic. Cluster II is part of an international programme to investigate how the Sun and Earth interact. The four satellites will join an armada of spacecraft from many countries (including ESA's SOHO satellite) which are already studying the Sun and the high-speed wind of charged particles - mainly electrons and protons - which it continually blasts into space.     The ESA micro satellite PROBA2 was launched on November 2, 2009. It carries two solar instruments, the radiometer LYRA and the coronal imager SWAP whose commissioning ended in March 2010. LYRA (PI: J.-F. Hochedez) observes the solar irradiance in 4 wavelengths chosen for their relevance to space weather, solar physics and Earth aeronomy. LYRA is able to follow the time evolution of solar flares at very high temporal resolution. SWAP (PI: D. Berghmans) takes an image of the EUV corona of the sun every minute in an extended field of view. SWAP is able to image all space weather significant events such as flares, coronal holes, dimmings, etc. We will present the technical capabilities of the two instruments and show their complementarity with e.g. SDO.     In support of the NRC-funded Nuclear Plant Aging Research (NPAR) program, Oak Ridge National Laboratory (ORNL) has carried out a comprehensive aging assessment of Motor-Operated Valves (MOVs). As part of this work, ORNL participated in the Gate Valve Flow Interruption Blowdown (GVFIB) tests carried out in Huntsville, Alabama. The GVFIB tests were intended primarily to determine the behavior of motor-operated gate valves under the temperature, pressure, and flow conditions expected to be experienced by isolation valves in Boiling Water Reactors (BWRs) during a high energy line break (blowdown) outside of containment. In addition, the tests provided an excellent opportunity to evaluate signature analysis methods for determining the operational readiness of the MOVs under those accident conditions. ORNL acquired motor current and torque switch shaft angular position data on two test MOVs during various times of the GVFIB tests. The reduction in operating ''margin'' of both MOVs due to the presence of additional valve running loads imposed by high flow was clearly observed in motor current and torque switch angular position signatures. In addition, the effects of differential pressure, fluid temperature, and line voltage on MOV operations were observed and more clearly understood as a result of utilizing signature analysis techniques. 1 ref.; 16 figs.

    Stack monitors are used to sense radioactive particulates and gases in effluent air being vented from rooms of nuclear facilities. These monitors record the levels and types of effluents to the environment. This paper presents the results of a stack monitor operating experience review of the U.S. Department of Energy (DOE) Occurrence Reporting and Processing System (ORPS) database records from the past 18 years. Regulations regarding these monitors are briefly described. Operating experiences reported by the U.S. DOE and in engineering literature sources were reviewed to determine the strengths and weaknesses of these monitors. Electrical faults, radiation instrumentation faults, and human errors are the three leading causes of failures. A representative “all modes” failure rate is 1E-04/hr. Repair time estimates vary from an average repair time of 17.5 hours (with spare parts on hand) to 160 hours (without spare parts on hand). These data should support the use of stack monitors in any nuclear facility, including the National Ignition Facility and the international ITER project.

    Background: A systematic review characterised and synthesised applications of the Community Readiness Tool (CRT) and synthesised quantitative results for readiness applications at multiple time points. Methods: Eleven databases in OvidSP and EBSCHOhost were searched to retrieve CRT applications. Information from primary studies was extracted independently by two researchers. Results: Forty applications of the CRT met inclusion criteria focussing on 14 different health and social issues. The community of interest was most often defined solely on the basis of its geographical location (52.5%). Most studies used the CRT to plan (85%) and/or evaluate programs (40%). The CRT protocol was modified in 40% of studies. Six applications evaluated readiness at multiple time points, however limited reporting in primary studies precluded any synthesis of results. Applications identified methodological rigour, contextual information and community engagement as strengths, and time and resource costs as limitations. Conclusions: The CRT is well suited for planning and evaluating complex community health interventions given its flexibility to accommodate diverse definitions of community and issues. CRT applications would benefit from improved reporting; reporting recommendations for use of the CRT are outlined. PMID:25811769

  7. Application of diagnostics to determine motor-operated valve operational readiness

    ORNL has been carrying out an aging assessment of motor-operated valves (MOVs) with the primary objective of recommending diagnostic methods for detecting and trending aging. As a result of experimental investigations at ORNL, it was discovered that the motor current during a valve stroke was a very useful diagnostic parameter for detecting and trending many MOV drive train load variations. The motor curent signatures were analyzed at four levels: mean value for a stroke, gross trends during a stroke, transients, and noise frequency spectra. Examples illustrating the use of this technique are presented. The use of motor current signature analysis was also shown to apply to other electric motor driven equipment. Future work includes developing a data base of MOV diagnostics, including criteria for determining the extent of degradation and application of the technique to other LWR motor driven safety equipment.

    The Telescope Data Center at Smithsonian Astrophysical Observatory supports observations and data flow from the optical telescopes of the Harvard-Smithsonian Center for Astrophysics. With a limited amount of manpower, we handle the entire process from instrument operations to the data archiving and scientific reduction, which allows us to avoid many of the difficulties other data centers encounter arising from the incompatibility of on the instrument and data processing sides. We use telescope control software and data reduction pipelines developed inside our unit which also helps us to ensure the consistency of data products at different stages of processing. Presently we are working on a data archive solution using PostgreSQL and GAVO DaCHS which will allow us to: (1) distribute the raw data to PIs on a timescale of days from observations; (2) feed the data reduction pipelines automatically; (3) distribute reduced science ready data to the PIs; (4) provide access to raw and processed data products using Virtual Observatory interfaces such as ObsTAP, SIAP, SSAP after the proprietary period of the data has expired; (5) maintain a searchable record of all astronomical sources observed by the instruments, along with a catalog of basic observing conditions and telescope/instrument status associated with each exposure. I will present the data archive prototypes for the MMIRS multi-object spectrograph. Our operations model can be scaled up and adopted by large optical observatories.

  11. A Systematic Review of Instruments to Assess Organizational Readiness for Knowledge Translation in Health Care

    Background The translation of research into practices has been incomplete. Organizational readiness for change (ORC) is a potential facilitator of effective knowledge translation (KT). However we know little about the best way to assess ORC. Therefore, we sought to systematically review ORC measurement instruments. Methods We searched for published studies in bibliographic databases (Pubmed, Embase, CINAHL, PsychINFO, Web of Science, etc.) up to November 1st, 2012. We included publications that developed ORC measures and/or empirically assessed ORC using an instrument at the organizational level in the health care context. We excluded articles if they did not refer specifically to ORC, did not concern the health care domain or were limited to individual-level change readiness. We focused on identifying the psychometric properties of instruments that were developed to assess readiness in an organization prior to implementing KT interventions in health care. We used the Standards for Educational and Psychological Testing to assess the psychometric properties of identified ORC measurement instruments. Findings We found 26 eligible instruments described in 39 publications. According to the Standards for Educational and Psychological Testing, 18 (69%) of a total of 26 measurement instruments presented both validity and reliability criteria. The Texas Christian University –ORC (TCU-ORC) scale reported the highest instrument validity with a score of 4 out of 4. Only one instrument, namely the Modified Texas Christian University – Director version (TCU-ORC-D), reported a reliability score of 2 out of 3. No information was provided regarding the reliability and validity of five (19%) instruments. Conclusion Our findings indicate that there are few valid and reliable ORC measurement instruments that could be applied to KT in the health care sector.     Independent consultants met at Los Alamos, June 15 and 16, 1993, to review progress on the commissioning of the Integrated Test Stand (ITS) for DARHT and to provide DOE with technical input on readiness for construction of the first radiographic arm of DARHT. The consultants concluded that all milestones necessary for demonstrating the performance of the DARHT accelerator have been met and that the project is ready for construction to resume. The experimental program using ITS should be continued to quantify the comparison of experiment and theory, to test improvements on the injector insulator, and to better evaluate the interaction of the beam and the target.

  16. Impact of Birth Preparedness and Complication Readiness Interventions on Birth with a Skilled Attendant: A Systematic Review

    Background Increased preparedness for birth and complications is an essential part of antenatal care and has the potential to increase birth with a skilled attendant. We conducted a systematic review of studies to assess the effect of birth preparedness and complication readiness interventions on increasing birth with a skilled attendant. Methods PubMed, Embase, CINAHL and grey literature were searched for studies from 2000 to 2012 using a broad range of search terms. Studies were included with diverse designs and intervention strategies that contained an element of birth preparedness and complication readiness. Data extracted included population, setting, study design, outcomes, intervention description, type of intervention strategy and funding sources. Quality of the studies was assessed. The studies varied in BP/CR interventions, design, use of control groups, data collection methods, and outcome measures. We therefore deemed meta-analysis was not appropriate and conducted a narrative synthesis of the findings. Results Thirty-three references encompassing 20 different intervention programmes were included, of which one programmatic element was birth preparedness and complication readiness. Implementation strategies were diverse and included facility-, community-, or home-based services. Thirteen studies resulted in an increase in birth with a skilled attendant or facility birth. The majority of authors reported an increase in knowledge on birth preparedness and complication readiness. Conclusions Birth Preparedness and Complication Readiness interventions can increase knowledge of preparations for birth and complications; however this does not always correspond to an increase in the use of a skilled attendant at birth.     A recent report on criticality accidents in nuclear facilities indicates that human error played a major role in a significant number of incidents with serious consequences and that some of these human errors may be related to the emotional state of the individual. A pre-shift test to detect a deleterious emotional state could reduce the occurrence of such errors in critical operations. The effectiveness of pre-shift testing is a challenge because of the need to gather predictive data in a relatively short test period and the potential occurrence of learning effects due to a requirement for frequent testing. This report reviews the different types of reliability and validity methods and testing and statistical analysis procedures to validate measures of emotional state. The ultimate value of a validation study depends upon the percentage of human errors in critical operations that are due to the emotional state of the individual. A review of the literature to identify the most promising predictors of emotional state for this application is highly recommended.

  19. Technologies and mechanisms for safety control of ready-to-eat muscle foods: an updated review.

    Ready-to-eat (RTE) muscle foods refer to a general category of meat and poultry products that are fully cooked and consumable without reheating. These products, including whole and sliced pork, beef, turkey, chicken, and variety of meats, in the forms of ham, roast, rolls, sausage, and frankfurter, are widely available in the delicatessen section of retail stores or various food service outlets. However, difficulties in avoidance of contamination by foodborne pathogens, notably Listeria monocytogenes, during product postlethality repackaging render RTE meats labile to outbreaks. Accordingly, the USDA-FSIS has established processing guidelines and regulations, which are constantly updated, to minimize foodborne pathogens in RTE products. Technologies that complement good manufacturing practice have been developed to control RTE meat safety. Among them, various antimicrobial product formulations, postpackaging pasteurization (thermal and nonthermal), and antimicrobial packaging are being used. Through these efforts, outbreaks linked to RTE meat consumption have substantially reduced in recent years. However, the pervasive and virulent nature of L. monocytogenes and the possible presence of other cold-tolerant pathogens entail continuing developments of new intervention technologies. This review updates existing and emerging physical and chemical methods and their mode of action to inactivate or inhibit threatening microorganisms in RTE muscle foods. PMID:24754253

  20. Demonstration of AIRS Total Ozone Products to Operations to Enhance User Readiness

    Cyclogenesis is a key forecast challenge at operational forecasting centers such as WPC and OPC, so these centers have a particular interest in unique products that can identify key storm features. In some cases, explosively developing extratropical cyclones can produce hurricane force, non-convective winds along the East Coast and north Atlantic as well as the Pacific Ocean, with the potential to cause significant damage to life and property. Therefore, anticipating cyclogenesis for these types of storms is crucial for furthering the NOAA goal of a "Weather Ready Nation". Over the last few years, multispectral imagery (i.e. RGB) products have gained popularity among forecasters. The GOES-R satellite champion at WPC/OPC has regularly evaluated the Air Mass RGB products from GOES Sounder, MODIS, and SEVIRI to aid in forecasting cyclogenesis as part of ongoing collaborations with SPoRT within the framework of the GOES-R Proving Ground. WPC/OPC has used these products to identify regions of stratospheric air associated with tropopause folds that can lead to cyclogenesis and hurricane force winds. RGB products combine multiple channels or channel differences into multi-color imagery in which different colors represent a particular cloud or air mass type. Initial interaction and feedback from forecasters evaluating the legacy Air Mass RGBs revealed some uncertainty regarding what physical processes the qualitative RGB products represent and color interpretation. To enhance forecaster confidence and interpretation of the Air Mass RGB, NASA SPoRT has transitioned a total column ozone product from AIRS retrievals to the WPC/OPC. The use of legacy AIRS demonstrates future JPSS capabilities possible with CrIS or OMPS. Since stratospheric air can be identified by anomalous potential vorticity and warm, dry, ozone-rich air, hyperspectral infrared sounder ozone products can be used in conjunction with the Air Mass RGB for identifying the role of stratospheric air in explosive

    Contamination of ready-to-eat products with Listeria monocytogenes may occur at several stages before consumption. Accessibility to the public and relatively limited control interventions at retail and food service establishments (compared with the processing sector of the food industry) and the lack of a specific regulatory framework increase the likelihood of introduction of this pathogen into some foods in these establishments. This review is a compilation of available information on the incidence and transmission of L. monocytogenes through ready-to-eat products at the retail and food service level. The potential transmission of L. monocytogenes within retail and food service operations has been indicated in epidemiological investigations and by survey data. Potential sources of the organism in these operations include the environment, food handlers, and incoming raw ingredients or processed products that have become contaminated after the lethality treatment at the manufacturing facility. L. monocytogenes may be present at retail and food service establishments in various ready-to-eat products, both prepackaged and those packaged in the store, and occasionally at high concentrations. This issue dictates the need for development and application of effective control measures, and potential control approaches are discussed here. Good manufacturing practices, appropriate cleaning, sanitation and hygiene programs, and temperature control required for prevention or inhibition of growth of the pathogen to high levels are critical for control of L. monocytogenes in the retail and food service sector. A comprehensive food safety system designed to be functional in retail and food service operations and based on the philosophy of hazard analysis and critical control point systems and a series of sound prerequisite programs can provide effective control of L. monocytogenes in these environments. CAMs alert personnel of potential inhalation exposures to radionuclides and can also actuate room ventilation isolation for public and environmental protection. This paper presents the results of a CAM operating experience review of the DOE Occurrence Reporting and Processing System (ORPS) database from the past 18 years. Regulations regarding these monitors are briefly reviewed. CAM location selection and operation are briefly discussed. Operating experiences reported by the U.S. Department of Energy and in other literature sources were reviewed to determine the strengths and weaknesses of these monitors. Power losses, human errors, and mechanical issues cause the majority of failures. The average “all modes” failure rate is 2.65E-05/hr. Repair time estimates vary from an average repair time of 9 hours (with spare parts on hand) to 252 hours (without spare parts on hand). These data should support the use of CAMs in any nuclear facility, including the National Ignition Facility and the international ITER experiment.

The present article outlines an extensive programme implemented in the Royal Norwegian Navy for personnel deployed in international operations. The challenges involved in international operations bare similarities to onboard personnel in civilian maritime operations. The program utilised by the Royal Norwegian Navy is extensive and not immediately applicable to civilian maritime companies. However, elements of this program could be used with limited resources. Questionnaire based screening, before, during and at the end of a contract period could result in early detections of mental health problems and increased retaining of personnel. This should be done by health professionals. Early targeting of at risk personnel could prevent serious costs for the individual as well as the company. PMID:25231333

  13. Integrating Balloon and Satellite Operation Data Centers for Technology Readiness Assessment

    Stratospheric balloon-borne experiments have been one of the most effective ways to validate innovative space technology, taking the advantage of reduced development cycles and low cost in launching and operation. In Brazil, the National Institute for Space Research (INPE) has balloon and satellite ground infrastructures since the 1970´s and the 1990´s, respectively. In the recent past, a strategic approach was adopted on the modernization of balloon ground operation facilities for supporting the protoMIRAX experiment, an X-ray imaging telescope under development at INPE as a pathfinder for the MIRAX (Monitor e Imageador de Raios X) satellite mission. The strategic target was to reuse the SATellite Control System (SATCS), a software framework developed to control and monitor INPÉs satellites, for balloon operation. This paper presents the results of that effort and the new ongoing project, a computer-based framework named I2Bso, which strategic target is to Integrate INPÉs Balloon and Satellite Operation data centers. The I2Bso major purpose is to support the continuous assessment of an innovative technology after different qualification flights either on board balloons or satellites in order to acquire growing evidence for the technology maturity.

    This paper identifies a gap in the team science literature that considers intrapersonal indicators of collaboration as motivations and threats to participating in collaborative knowledge producing teams (KPTs). Through a scoping review process, over 150 resources were consulted to organize 6 domains of motivation and threat to collaboration in KPTs: Resource Acquisition, Advancing Science, Building Relationships, Knowledge Transfer, Recognition and Reward, and Maintenance of Beliefs. Findings show how domains vary in their presentation of depth and diversity of motivation and threat indicators as well as their relationship with each other within and across domains. The findings of 51 indicators resulting from the review provide a psychosocial framework for which to establish a hierarchy of collaborative reasoning for individual engagement in KPTs thus allowing for further research into the mechanism of collaborative engagement. The indicators serve as a preliminary step in establishing a protocol for testing of the psychometric properties of intrapersonal measures of collaboration readiness. PMID:27398411

    Monitoring the breathing air in tritium facility rooms for airborne tritium is a radiological safety requirement and a best practice for personnel safety. Besides audible alarms for room evacuation, these monitors often send signals for process shutdown, ventilation isolation, and cleanup system actuation to mitigate releases and prevent tritium spread to the environment. Therefore, these monitors are important not only to personnel safety but also to public safety and environmental protection. This paper presents an operating experience review of tritium monitor performance on demand during small (1 mCi to 1 Ci) operational releases, and intentional airborne inroom tritium release tests. The tritium tests provide monitor operation data to allow calculation of a statistical estimate for the reliability of monitors annunciating in actual tritium gas airborne release situations. The data show a failure to operate rate of 3.5E-06/monitor-hr with an upper bound of 4.7E-06, a failure to alarm on demand rate of 1.4E-02/demand with an upper bound of 4.4E-02, and a spurious alarm rate of 0.1 to 0.2/monitor-yr.

    Consumer health technologies can educate patients about diabetes and support their self-management, yet usability evidence is rarely published even though it determines patient engagement, optimal benefit of any intervention, and an understanding of generalizability. Therefore, we conducted a narrative review of peer-reviewed articles published from 2009–2013 that tested the usability of a web- or mobile-delivered system/application designed to educate and support patients with diabetes. Overall, the 23 papers included in our review used mixed (n=11), descriptive quantitative (n=9), and qualitative methods (n=3) to assess usability, such as documenting which features performed as intended and how patients rated their experiences. More sophisticated usability evaluations combined several complementary approaches to elucidate more aspects of functionality. Future work pertaining to the design and evaluation of technology-delivered diabetes education/support interventions should aim to standardize the usability testing processes, and publish usability findings to inform interpretation of why an intervention succeeded or failed, and for whom.   10. Interpreting systematic reviews: are we ready to make our own conclusions? A cross-sectional study

    Background Independent evaluation of clinical evidence is advocated in evidence-based medicine (EBM). However, authors' conclusions are often appealing for readers who look for quick messages. We assessed how well a group of Malaysian hospital practitioners and medical students derived their own conclusions from systematic reviews (SRs) and to what extent these were influenced by their prior beliefs and the direction of the study results. Methods We conducted two cross-sectional studies: one with hospital practitioners (n = 150) attending an EBM course in June 2008 in a tertiary hospital and one with final-year medical students (n = 35) in November 2008. We showed our participants four Cochrane SR abstracts without the authors' conclusions. For each article, the participants chose a conclusion from among six options comprising different combinations of the direction of effect and the strength of the evidence. We predetermined the single option that best reflected the actual authors' conclusions and labelled this as our best conclusion. We compared the participants' choices with our predetermined best conclusions. Two chosen reviews demonstrated that the intervention was beneficial ("positive"), and two others did not ("negative"). We also asked the participants their prior beliefs about the intervention. Results Overall, 60.3% correctly identified the direction of effect, and 30.1% chose the best conclusions, having identified both the direction of effect and the strength of evidence. More students (48.2%) than practitioners (22.2%) chose the best conclusions (P < 0.001). Fewer than one-half (47%) correctly identified the direction of effect against their prior beliefs. "Positive" SRs were more likely than "negative" SRs to change the participants' beliefs about the effect of the intervention (relative risk (RR) 1.8, 95% confidence interval 1.3 to 2.6) and "convert" those who were previously unsure by making them choose the appropriate direction of effect (RR 1

    Operating room (OR) managers must plan staffing in the face of uncertain demand for OR time. Planning too much staffing results in underutilized OR time. Planning too little staffing causes overutilized time, which is approximately twice as expensive as underutilized time. Deciding how much staffing to plan for an OR is analogous to the classic newsvendor problem in operations research. A newsvendor must decide how much product to order based on its cost c and sales price p, plus estimates of the uncertain future demand for the product. The newsvendor problem has a simple mathematical solution. The correct amount of product to order is the (p - c)/p quantile of the demand for the product. This optimal order quantity is analogous mathematically to the number of hours of OR time for which staffing should be planned. We performed a systematic review of the behavioral operations experimental literature on newsvendor problems relevant to OR management. Student volunteers participating in experimental studies have great difficulty knowing how much product to order, given c, p, and the demand distribution. Decision making is only modestly improved by more frequent feedback. Even scores of rounds of ordering are insufficient for much learning to occur. Suboptimal decisions result from innate psychological biases. Students anchor on mean demand, make insufficient adjustments, and rely disproportionately on the most recent demand values. The behavior of OR managers who plan staffing for the OR is analogous to that of students participating in a newsvendor experiment. Month after month, an OR manager will plan too little staffing for the surgeon who consistently ends the day late and too much staffing for the surgeon who consistently does not fill an OR. Experimental studies of the newsvendor problem provide mechanistic insights into the reasons that OR managers make poor decisions when planning OR staffing.     We have carried out experiments to explore how different vacuum magnetic-field configurations affect plasma performance on SSPX. Six independently controllable magnetic field coils and power supplies can be programmed to obtain a wide range of field line configurations: those with vacuum magnetic field lines passing parallel to the side walls of the SSPX chamber and those with diverging fields passing through the side walls of the chamber. The new coils have greatly expanded the density range over which good spheromak plasmas are observed. This results from the ability to form, using the bias coils, a Penning discharge so that spheromak formation can be obtained at lower input gas pressure. In addition, spheromak theory suggests an increase in field amplification could be expected with magnetic field lines parallel to the chamber walls. We observed about a 20% increase in field amplification suggesting that current flow during formation and the resultant MHD stability play an important role in magnetic field build up. Energy confinement and magnetic field generation during operation with the bias coils will be presented.     This report presents a review of cryogenic system operating experiences, from particle accelerator, fusion experiment, space research, and other applications. Safety relevant operating experiences and accident information are discussed. Quantitative order-of-magnitude estimates of cryogenic component failure rates and accident initiating event frequencies are presented for use in risk assessment, reliability, and availability studies. Safety concerns with cryogenic systems are discussed, including ozone formation, effects of spills, and modeling spill behavior. This information should be useful to fusion system designers and safety analysts, such as the team working on the International Thermonuclear Experimental Reactor design.

  11. Review of the IEEE Standard for Computerized Operating Procedure Systems

    Increasingly nuclear power plant procedures, such as emergency operating procedures, are being presented in computer form with functionality to support operator use and management of the procedures. The U.S. Nuclear Regulatory Commission (NRC) currently has guidance for the review of computer-based procedures (CBPs); however, there remain CBP functions and human performance issues for which up-to-date guidance is lacking. The Institute of Electrical and Electronics Engineers (IEEE) has initiated a standard development effort to address the human factors engineering (HFE) aspects of CBP systems. When completed, it may provide guidance to supplement the NRC staff's review criteria. The purpose of our study was to evaluate the suitability of the IEEE Standard for use in the NRC's HFE safety reviews of CBP systems and to ensure that the guidance meets the NRC's standard for scientific and engineering rigor used in its own guidance development efforts. We established the following criteria with which to evaluate the Standard: (1) it should meet an existing need of NRC reviewers, (2) it should be based in sound HFE principles, (3) it should be thoroughly peer-reviewed, and (4) it should address CBP-related human performance issues identified in the literature. This report describes the methodology we used to evaluate each criterion. Our evaluation concluded that the Standard generally does meet these criteria, however several areas were identified for which additional clarifications are needed. Thus consideration of the Standard's use by the NRC is supported. The standard evaluation methodology developed in this study can be generally applied to the review of other HFE standards being considered for possible use or endorsement by the NRC.

    This report is a collection and review of system operation and failure experiences for air ventilation systems in nuclear facilities. These experiences are applicable for magnetic and inertial fusion facilities since air ventilation systems are support systems that can be considered generic to nuclear facilities. The report contains descriptions of ventilation system components, operating experiences with these systems, component failure rates, and component repair times. Since ventilation systems have a role in mitigating accident releases in nuclear facilities, these data are useful in safety analysis and risk assessment of public safety. An effort has also been given to identifying any safety issues with personnel operating or maintaining ventilation systems. Finally, the recommended failure data were compared to an independent data set to determine the accuracy of individual values. This comparison is useful for the International Energy Agency task on fusion component failure rate data collection.

    Nankai Trough Seismogenic Zone Experiment (NanTroSEIZE) Stage 1 is first step in the ambitious, coordinated multistage, multi-expedition project from the Integrated Ocean Drilling Program (IODP). Through out the 138 days of continuous operations in three expeditions, two major challenges caused various problems and delays. The Kuroshio current presented at all sites and more often at 3 knot or greater velocity, and the difficult borehole conditions in this tectonically active faulting environment obviously caused substantial loss of operational time, failure of equipments and loss of toolstring. Several other technical problems, ranging from dynamic positioning system, remotely operated vehicle (ROV) to wait-on-weather, made additional delays as well. During the course of these expeditions, Chikyu was able to test and refine operational techniques and structures, and able to pioneer the use of new tools and equipments not only in the lab area but also at the rig floor coring and logging operations. Those include first-ever and rarely-used logging-while-drilling (LWD) tools in the scientific drilling history, new coring systems and techniques, extensive use of X-ray Computed Tomography (XCT) on whole core sections and several others. Expedition 314 collected 4274 m of LWD logs and conducted 2285.5 m of Measurement-While-Drilling (MWD) logs from pilot holes. Expedition 315 recovered 808 m of core from 1287 m penetration, and Expedition 316 recovered 1340 m of core out of 2103 m penetration. With intention of improving CDEX's expedition planning and science services in the future, major review work is in progress focusing on operational, technological and science services. Stennis Space Center Deputy Director Patrick Scheuermann with a commemorative plate designating the facility as a Project Ready technology park during a Nov. 6 ceremony. The designation certifies Stennis as a 'project ready' site for high-tech firms seeking a new location. Mississippi Power launched the Project Ready Certified Site Program to identify locations that are ready to go and relatively risk-free in an effort to attract companies into the region. Stennis is the first site to gain the technology park certification.   6. Getting a technology-based diabetes intervention ready for prime time: a review of usability testing studies.

    Consumer health technologies can educate patients about diabetes and support their self-management, yet usability evidence is rarely published even though it determines patient engagement, optimal benefit of any intervention, and an understanding of generalizability. Therefore, we conducted a narrative review of peer-reviewed articles published from 2009 to 2013 that tested the usability of a web- or mobile-delivered system/application designed to educate and support patients with diabetes. Overall, the 23 papers included in our review used mixed (n = 11), descriptive quantitative (n = 9), and qualitative methods (n = 3) to assess usability, such as documenting which features performed as intended and how patients rated their experiences. More sophisticated usability evaluations combined several complementary approaches to elucidate more aspects of functionality. Future work pertaining to the design and evaluation of technology-delivered diabetes education/support interventions should aim to standardize the usability testing processes and publish usability findings to inform interpretation of why an intervention succeeded or failed and for whom.   7. TOPICAL REVIEW: Strategies for increasing the operating frequency range of vibration energy harvesters: a review

    This review presents possible strategies to increase the operational frequency range of vibration-based micro-generators. Most vibration-based micro-generators are spring-mass-damper systems which generate maximum power when the resonant frequency of the generator matches the frequency of the ambient vibration. Any difference between these two frequencies can result in a significant decrease in generated power. This is a fundamental limitation of resonant vibration generators which restricts their capability in real applications. Possible solutions include the periodic tuning of the resonant frequency of the generator so that it matches the frequency of the ambient vibration at all times or widening the bandwidth of the generator. Periodic tuning can be achieved using mechanical or electrical methods. Bandwidth widening can be achieved using a generator array, a mechanical stopper, nonlinear (e.g. magnetic) springs or bi-stable structures. Tuning methods can be classified into intermittent tuning (power is consumed periodically to tune the device) and continuous tuning (the tuning mechanism is continuously powered). This review presents a comprehensive review of the principles and operating strategies for increasing the operating frequency range of vibration-based micro-generators presented in the literature to date. The advantages and disadvantages of each strategy are evaluated and conclusions are drawn regarding the relevant merits of each approach.

  8. Ready for takeoff? A critical review of armodafinil and modafinil for the treatment of sleepiness associated with jet lag

    Jet lag syndrome (JLS) is a clinical syndrome of disrupted nocturnal sleep and daytime neurocognitive impairment which occurs in the context of rapid transmeridian travel. Many strategies for treatment of JLS exist, and include hypnotics to enhance nocturnal sleep, chronotherapeutic approaches (eg, light therapy, melatonin, or gradual schedule shifting), and alerting agents to counter daytime sleepiness. Safety concerns have prompted renewed interest in managing JLS-associated excessive daytime sleepiness (JLSAEDS). Off-label use of the newer alerting agents modafinil and armodafinil is increasing for this indication, often at the specific request of patients. In order to better evaluate the potential risks and benefits of these medications for the management of JLSAEDS, clinicians must be aware of what is known – and still not known. In this article, the pharmacology and pharmacokinetics of modafinil and armodafinil are reviewed, along with evidence for their efficacy in treating sleepiness associated with narcolepsy, obstructive sleep apnea and shift work sleep disorder. Clinical trial data for use of alerting agents in the management of JLSAEDS are limited to one three-day trial involving armodafinil, dosed in the morning to treat JLSAEDS in the setting of eastbound transmeridian travel. This study showed improvement in objective measures of daytime sleepiness at doses of 50 and 150 mg per day. However, global impression of clinical severity of symptom scores only improved on day 1 for those patients receiving 150 mg, and were otherwise not superior to placebo. Consideration for the use of modafinil or armodafinil for the treatment of sleepiness associated with JLS involves careful integration of patient-reported goals, a review of medical contraindications, and an awareness of rare adverse events. More research is needed in order to identify those who are most likely to benefit from this intervention and better define the risk-benefit ratio for this

    The Office of Special Projects (DP-35), formerly Office of Self-Assessment (DP-9), analyzed occurrences caused by problems with equipment and material and recommended the following systems for an in-depth study: (1) Selective Alpha Air Monitor (SAAM), (2) Emergency Diesel Generator, (3) Ventilation System, (4) Fire Alarm System. Further, DP-35 conducted an in-depth review of the problems associated with SAAM and with diesel generators, and made several recommendations. This study focusses on ventilation system. The intent was to determine the causes for the events related to these system that were reported in the Occurrence Reporting and Processing System (ORPS), to identify components that failed, and to provide technical information from the commercial and nuclear industries on the design, operation, maintenance, and surveillance related to the system and its components. From these data, sites can develop a comprehensive program of maintenance management, including surveillance, to avoid similar occurrences, and to be in compliance with the following DOE orders.

  12. Occipitocervical Fusion Surgery: Review of Operative Techniques and Results.

    Objective Varying types of clinicoradiologic presentations at the craniovertebral junction (CVJ) influence the decision process for occipitocervical fusion (OCF) surgery. We discuss the operative techniques and decision-making process in OCF surgery based on our clinical experience and a literature review. Material and Methods A total of 49 consecutive patients who underwent OCF participated in the study. Sagittal computed tomography images were used to illustrate and measure radiologic parameters. We measured Wackenheim clivus baseline (WCB), clivus-canal angle (CCA), atlantodental distance (ADD), and Powers ratio (PR) in all the patients. Results Clinical improvement on Nurick grading was recorded in 36 patients. Patients with better preoperative status (Nurick grades 1-3) had better functional outcomes after the surgery (p = 0.077). Restoration of WCB, CCA, ADD, and PR parameters following the surgery was noted in 39.2%, 34.6%, 77.4%, and 63.3% of the patients, respectively. Complications included deep wound infections (n = 2), pseudoarthrosis (n = 2), and deaths (n = 4). Conclusion Conventional wire-based constructs are superseded by more rigid screw-based designs. Odontoidectomy is associated with a high incidence of perioperative complications. The advent of newer implants and reduction techniques around the CVJ has obviated the need for this procedure in most patients. PMID:26401473

  7. Challenges to the Programmatic Implementation of Ready to Use Infant Formula in the Post-Earthquake Response, Haiti, 2010: A Program Review

    Background and Objectives Following the 2010 earthquake in Haiti, infant and young child feeding was identified as a priority nutrition intervention. A new approach to support breastfeeding mothers and distribute ready-to-use infant formula (RUIF) to infants unable to breastfeed was established. The objective of the evaluation was to assess the implementation of infant feeding programs using RUIF in displaced persons camps in Port-au-Prince, Haiti during the humanitarian response. Methods A retrospective record review was conducted from April–July, 2010 to obtain data on infants receiving RUIF in 30 baby tents. A standardized data collection form was created based on data collected across baby tents and included: basic demographics, admission criteria, primary caretaker, feeding practices, and admission and follow-up anthropometrics. Main Findings Orphans and abandoned infants were the most frequent enrollees (41%) in the program. While the program targeted these groups, it is unlikely that this is a true reflection of population demographics. Despite programmatic guidance, admission criteria were not consistently applied across programs. Thirty-four percent of infants were undernourished (weight for age Z score <−2) at the time of admission. Defaulting accounted for 50% of all program exits and there was no follow-up of these children. Low data quality was a significant barrier. Conclusions The design, implementation and magnitude of the ‘baby tents’ using RUIF was novel in response to infant and young child feeding (IYCF) in emergencies and presented multiple challenges that should not be overlooked, including adherence to protocols and the adaption of emergency programs to existing programs. The implementation of IYCF programs should be closely monitored to ensure that they achieve the objectives set by the humanitarian community and national government.     The development of new system capabilities typically depends upon the prior success of advanced technology research and development efforts. These systems developments inevitably face the three major challenges of any project: performance, schedule and budget. Done well, advanced technology programs can substantially reduce the uncertainty in all three of these dimensions of project management. Done poorly, or not at all, and new system developments suffer from cost overruns, schedule delays and the steady erosion of initial performance objectives. It is often critical for senior management to be able to determine which of these two paths is more likely—and to respond accordingly. The challenge for system and technology managers is to be able to make clear, well-documented assessments of technology readiness and risks, and to do so at key points in the life cycle of the program. In the mid 1970s, the National Aeronautics and Space Administration (NASA) introduced the concept of "technology readiness levels" (TRLs) as a discipline-independent, programmatic figure of merit (FOM) to allow more effective assessment of, and communication regarding the maturity of new technologies. In 1995, the TRL scale was further strengthened by the articulation of the first definitions of each level, along with examples (J. Mankins, Technology readiness levels, A White Paper, NASA, Washington, DC, 1995. [1]). Since then, TRLs have been embraced by the U.S. Congress' General Accountability Office (GAO), adopted by the U.S. Department of Defense (DOD), and are being considered for use by numerous other organizations. Overall, the TRLs have proved to be highly effective in communicating the status of new technologies among sometimes diverse organizations. This paper will review the concept of "technology readiness assessments", and provide a retrospective on the history of "TRLs" during the past 30 years.     An earlier investigation by academic and community co-investigators led to the development of the Partnership Readiness for Community-Based Participatory Research (CBPR) Model, which defined major dimensions and key indicators of partnership readiness. As a next step in this process, we used qualitative methods, cognitive pretesting, and expert reviews to develop a working guide, or toolkit, based on the model for academic and community partners to assess and leverage their readiness for CBPR. The 75-page toolkit is designed as a qualitative assessment promoting equal voice and transparent, bi-directional discussions among all the partners. The toolkit is formatted to direct individual partner assessments, followed by team assessments, discussions, and action plans to optimize their goodness of fit, capacity, and operations to conduct CBPR. The toolkit has been piloted with two cohorts in the Medical University of South Carolina’s (MUSC) Community Engaged Scholars (CES) Program with promising results from process and outcome evaluation data.   12. Protective lung ventilation in operating room: a systematic review.

    Postoperative pulmonary and extrapulmonary complications adversely affect clinical outcomes and healthcare utilization, so that prevention has become a measure of the quality of perioperative care. Mechanical ventilation is an essential support therapy to maintain adequate gas exchange during general anesthesia for surgery. Mechanical ventilation using high tidal volume (VT) (between 10 and 15 mL/kg) has been historically encouraged to prevent hypoxemia and atelectasis formation in anesthetized patients undergoing abdominal and thoracic surgery. However, there is accumulating evidence from both experimental and clinical studies that mechanical ventilation, especially the use of high VT and plateau pressure, may potentially aggravate or even initiate lung injury. Ventilator-associated lung injury can result from cyclic alveolar overdistension of non-dependent lung tissue, and repetitive opening and closing of dependent lung tissue resulting in ultrastructural damage at the junction of closed and open alveoli. Lung-protective ventilation, which refers to the use of lower VT and limited plateau pressure to minimize overdistension, and positive end-expiratory pressure to prevent alveolar collapse at end-expiration, was shown to improve outcome in critically ill patients with acute respiratory distress syndrome (ARDS). It has been recently suggested that this approach might also be beneficial in a broader population, especially in critically ill patients without ARDS at the onset of mechanical ventilation. There is, however, little evidence regarding a potential beneficial effect of lung protective ventilation during surgery, especially in patients with healthy lungs. Although surgical patients are frequently exposed to much shorter periods of mechanical ventilation, this is an important gap in knowledge given the number of patients receiving mechanical ventilation in the operating room.   1. Birth Preparedness and Complication Readiness (BPCR) interventions to reduce maternal and neonatal mortality in developing countries: systematic review and meta-analysis

    Background Birth Preparedness and Complication Readiness (BPCR) interventions are widely promoted by governments and international agencies to reduce maternal and neonatal health risks in developing countries; however, their overall impact is uncertain, and little is known about how best to implement BPCR at a community level. Our primary aim was to evaluate the impact of BPCR interventions involving women, families and communities during the prenatal, postnatal and neonatal periods to reduce maternal and neonatal mortality in developing countries. We also examined intervention impact on a variety of intermediate outcomes important for maternal and child survival. Methods We conducted a systematic review and meta-analysis of randomized trials of BPCR interventions in populations of pregnant women living in developing countries. To identify relevant studies, we searched the scientific literature in the Pubmed, Embase, Cochrane library, Reproductive health library, CINAHL and Popline databases. We also undertook manual searches of article bibliographies and web sites. Study inclusion was based on pre-specified criteria. We synthesised data by computing pooled relative risks (RR) using the Cochrane RevMan software. Results Fourteen randomized studies (292 256 live births) met the inclusion criteria. Meta-analyses showed that exposure to BPCR interventions was associated with a statistically significant reduction of 18% in neonatal mortality risk (twelve studies, RR = 0.82; 95% CI: 0.74, 0.91) and a non-significant reduction of 28% in maternal mortality risk (seven studies, RR = 0.72; 95% CI: 0.46, 1.13). Results were highly heterogeneous (I2 = 76%, p < 0.001 and I2 = 72%, p = 0.002 for neonatal and maternal results, respectively). Subgroup analyses of studies in which at least 30% of targeted women participated in interventions showed a 24% significant reduction of neonatal mortality risk (nine studies, RR = 0.76; 95% CI: 0.69, 0.85) and a

  12. A review of proposed Glen Canyon Dam interim operating criteria

    Three sets of interim operating criteria for Glen Canyon Dam on the Colorado River have been proposed for the period of November 1991, to the completion of the record of decision for the Glen Canyon Dam environmental impact statement (about 1993). These criteria set specific limits on dam releases, including maximum and minimum flows, up-ramp and down-ramp rates, and maximum daily fluctuation. Under the proposed interim criteria, all of these parameters would be reduced relative to historical operating criteria to protect downstream natural resources, including sediment deposits, threatened and endangered fishes, trout, the aquatic food base, and riparian plant communities. The scientific bases of the three sets of proposed operating criteria are evaluated in the present report:(1) criteria proposed by the Research/Scientific Group, associated with the Glen Canyon Environmental Studies (GCES); (2) criteria proposed state and federal officials charged with managing downstream resources; and (3) test criteria imposed from July 1991, to November 1991. Data from Phase 1 of the GCES and other sources established that the targeted natural resources are affected by dam operations, but the specific interim criteria chosen were not supported by any existing studies. It is unlikely that irreversible changes to any of the resources would occur over the interim period if historical operating criteria remained in place. It is likely that adoption of any of the sets of proposed interim operating criteria would reduce the levels of sediment transport and erosion below Glen Canyon Dam; however, these interim criteria could result in some adverse effects, including the accumulation of debris at tributary mouths, a shift of new high-water-zone vegetation into more flood-prone areas, and further declines in vegetation in the old high water zone.

    Monitoring tritium facility and fusion experiment effluent streams is an environmental safety requirement. This paper presents data on the operating experience of a solid scintillant monitor for tritium in effluent water. Operating experiences were used to calculate an average monitor failure rate of 4E-05/hour for failure to function. Maintenance experiences were examined to find the active repair time for this type of monitor, which varied from 22 minutes for filter replacement to 11 days of downtime while waiting for spare parts to arrive on site. These data support planning for monitor use; the number of monitors needed, allocating technician time for maintenance, inventories of spare parts, and other issues.

    Background: Concept of e-readiness is used in many areas such as e-business, e-commerce, e-government, and e-banking. In terms of healthcare, e-readiness is a rather new concept, and is propounded under the title of E-healthcare. E-health readiness refers to the readiness of communities and healthcare institutions for the expected changes brought by programs related to Information and Communications Technology (lCT). The present research is conducted aiming at designing E-health Readiness Assessment Framework (EHRAF) in Iran. Methods: The e-health readiness assessment framework was designed based on reviewing literature on e-readiness assessment models and opinions of ICT and health experts. In the next step, Delphi method was used to develop and test the designed framework. Three questionnaires developed to test and modify the model while determining weights of the indices; afterward they were either sent to experts through email or delivered to them in face. Results: The designed framework approved with 4 dimensions, 11 constituents and 58 indices. Technical readiness had the highest importance coefficient (0.256099), and the other dimensions were of the next levels of coefficient importance: core readiness (0.25520), social communication readiness (0.244658), and engagement readiness (0.244039). Conclusion: The framework presents the movement route and investment priorities in e-health in Iran. The proposed framework is a good instrument for measuring the e-readiness in health centers in Iran, and for identifying strengths and weaknesses of these centers to access ICT and its implementation for more effectiveness and for analyzing digital divide between them, as well.     The National Oceanic and Atmospheric Administration's (NOAA) National Weather Service (NWS), in collaboration with the NOAA National Environmental Satellite, Data and Information Service (NESDIS) and its Cooperative Institutes, have been prototyping various operational applications of Suomi-NPP satellite imagery and products. Some of these new satellite capabilities are NOAA and S-NPP mission unique and have resulted in new science applications for high impact events and related impact-based decision support services. From detection to monitoring to recovery-phase operations, S-NPP debuts new NOAA-unique capabilities for true color RGB imagery, Near Constant Contrast Day-Night Band Imagery, Flood/Ice Detection and Monitoring, Wildfire and Smoke Detection and Monitoring, Severe Weather Environmental and Storm Analysis, Dust Detection and Monitoring, and Global Infrared and Microwave Atmospheric Soundings.     There is currently a pressing need for safe, reliable, cost-effective methods of evaluating driving ability. With recent improvements in virtual reality technology, driving simulators seem to offer a promising alternative to on-road methods of driving assessment. One population at risk for driving difficulties may be veterans returning from combat in Iraq or Afghanistan. The use of driving simulators to evaluate and remediate veterans' abilities to operate a motor vehicle is a rehabilitative goal. However, there are no consistent standardized procedures for determining safe from unsafe driving using driving simulators, which limit the clinical utility of this important tool. The purposes of this article are (1) to give the reader a better understanding of the parameters that are most commonly measured in the driving simulation literature and (2) to review parameters that are most relevant for the Operation Enduring Freedom/Operation Iraqi Freedom veteran population.     A parallel readiness assessment (RA) was conducted by independent Fluor Hanford (FH) and U. S. Department of Energy, Richland Operations Office (RL) team to verify that an adequate state of readiness had been achieved for activities associated with the packaging and shipping of pressurized water reactor fuel assemblies from B-Cell in the 324 Building to the interim storage area at the Canister Storage Building in the 200 Area. The RL review was conducted in parallel with the FH review in accordance with the Joint RL/FH Implementation Plan (Appendix B). The RL RA Team members were assigned a FH RA Team counterpart for the review. With this one-on-one approach, the RL RA Team was able to assess the FH Team's performance, competence, and adherence to the implementation plan and evaluate the level of facility readiness. The RL RA Team agrees with the FH determination that startup of the 324 Building B-Cell pressurized water reactor spent nuclear fuel packaging and shipping operations can safely proceed, pending completion of the identified pre-start items in the FH final report (see Appendix A), completion of the manageable list of open items included in the facility's declaration of readiness, and execution of the startup plan to operations.

  10. Use of pre-operative steroids in liver resection: a systematic review and meta-analysis

    Background By attenuating the systemic inflammatory response to major surgery, the pre-operative administration of steroids may reduce the incidence of complications. Methods A systematic review was conducted to identify randomized controlled trials (RCT) comparing pre-operative steroid administration with placebo during a liver resection. Meta-analyses were performed. Results Five RCTs were identified including a total of 379 patients. Pre-operative steroids were associated with statistically significant reductions in the levels of serum bilirubin and interleukin 6 (IL-6) on post-operative day one. There was a trend towards a lower incidence of post-operative complications and prothrombin time (PT), but this did not reach statistical significance. Conclusion Pre-operative steroids may be associated with a clinically significant benefit in liver resection. PMID:23461716

    At the request of the President of the United States through the Office of Science and Technology Policy (OSTP), the NASA Administrator tasked the Aerospace Safety Advisory Panel with the responsibility to identify and review issues associated with the safe operation and management of the Space Shuttle program arising from ongoing efforts to improve and streamline operations. These efforts include the consolidation of operations under a single Space Flight Operations Contract (SFOC), downsizing the Space Shuttle workforce and reducing costs of operations and management. The Panel formed five teams to address the potentially significant safety impacts of the seven specific topic areas listed in the study Terms of Reference. These areas were (in the order in which they are presented in this report): Maintenance of independent safety oversight; implementation plan for the transition of Shuttle program management to the Lead Center; communications among NASA Centers and Headquarters; transition plan for downsizing to anticipated workforce levels; implementation of a phased transition to a prime contractor for operations; Shuttle flight rate for Space Station assembly; and planned safety and performance upgrades for Space Station assembly. The study teams collected information through briefings, interviews, telephone conversations and from reviewing applicable documentation.     If, despite risks to workers and these potential impacts, stakeholders decide that P4 wastes need to be excavated and treated, the Review Team determined that a number of the ETTs examined warrant further consideration for the treatment of P4 waste that has been characterized (for example, P4 waste present in the historical ponds). Nevertheless, concerns about the health and safety of site investigation workers using then-available investigation approaches prevented the collection of subsurface samples containing P4 from large areas of the site (e.g., the railroad swale, the vadose zone beneath the Furnace Building, and the abandoned railcars), As a result, the contaminant CSM in those particular areas was not refined enough to allow the Review Team to draw conclusions about using some of the ETTs to treat P4 waste in those areas. The readiness of an ETT for implementation varies depending on many factors, including stakeholder input, permitting, and remedial action construction requirements. Technologies that could be ready for use in the near term (within 1 year) include the following: mechanical excavation, containment technologies, off-site incineration, and drying and mechanical mixing under a tent structure. Technologies that could be ready for use in the mid-term (1 to 2 years) include cutter suction dredging, thermal-hydraulic dredging, and underground pipeline cleaning technologies. Technologies requiring a longer lead time (2 to 5 years) include on-site incineration, a land disposal restriction waste treatment system, an Albright & Wilson batch mud still, post-treatment on-site disposal, and post-treatment off-site disposal.

    Constructed wetlands (CWs) have been used as a green technology to treat various wastewaters for several decades. CWs offer a land-intensive, low-energy, and less-operational-requirements alternative to conventional treatment systems, especially for small communities and remote locations. However, the sustainable operation and successful application of these systems remains a challenge. Hence, this paper aims to provide and inspire sustainable solutions for the performance and application of CWs by giving a comprehensive review of CWs' application and the recent development on their sustainable design and operation for wastewater treatment. Firstly, a brief summary on the definition, classification and application of current CWs was presented. The design parameters and operational conditions of CWs including plant species, substrate types, water depth, hydraulic load, hydraulic retention time and feeding mode related to the sustainable operation for wastewater treatments were then discussed. Lastly, future research on improving the stability and sustainability of CWs were highlighted. PMID:25453440

    The prelaunch tests for the Space Transportation System 1 flight are briefly described. Testing is divided into two major sections: the wet countdown demonstration test/flight readiness firing, which includes a 20 second test firing of the orbiter's three main engines, and a mission verification test, which is centered on flight and landing operations. The functions of the countdown sequence are listed and end of mission and mission abort exercises are described.
