  1. The revitalization of the Public Health Service Commissioned Corps.

    The Public Health Service (PHS) is the second oldest uniformed service of the United States; its tradition commenced with the establishment of the Marine Hospital Service in 1798. Congress, in 1889, established the United States Public Health Service Commissioned Corps under the aegis of the Treasury. The Corps was created as a uniformed nonmilitary service with a distinct uniform, insignia, and with titles, pay, and retirement protocols that corresponded to those of the uniformed military services (the Armed Forces). Initially the health care system of the country, and Commissioned Corps members, were concerned with infectious and vitamin-deficiency diseases; more recently the nation's medical community has focused on cardiovascular diseases, cancer, and AIDS. A comprehensive revitalization of the Commissioned Corps began in April 1987. The intent was to restore the Commissioned Corps to its traditional leadership role as a cadre of mobile, compassionate experts ensuring the nation's health. The revitalization activities have been successful. The Commissioned Corps has approximately 5,500 active duty officers. The Surgeon General directed the development of career tracks for 11 categories of commissioned officers to increase the opportunities for professional development within the PHS and thus increase retention and professional growth. The theme for the 1989 celebration of the centennial of the Commissioned Corps is "a century of service with distinction." A hundred years from now, at the bicentennial of the Commissioned Corps, the current Surgeon General would like it to be said that the Public Health Service has had "two centuries of service with distinction."   4. The U.S. Public Health Service Commissioned Corps: a need for engineers.

    In 1798, President John Adams signed the bill creating the Marine Hospital Service to medically care for U.S. merchant seamen. That agency is known today as the U.S. Public Health Service (PHS). In 1918, the PHS Commissioned Corps commissioned sanitary engineers in the Reserve Corps, and in 1930, President Herbert Hoover signed the Parker Act authorizing sanitary engineers to be part of the regular Commissioned Corps. During the world war years, PHS engineers were detailed to improve sanitation for military bases in the United States and abroad. In the 1960s, most PHS engineers served in the Indian Health Service, upgrading sanitation facilities for American Indians and Alaskan Natives. During the 1970s, PHS engineers were involved in providing an integrated and coordinated attack on environmental issues. Today, PHS engineers are involved with many aspects of public health protection and have been detailed around the world to provide emergency assistance in the wake of disasters. PMID:11149068

  7. Personalized commissioning, public spaces: the limits of the market in English social care services

    Background The article explores the implications of personal budgets within English social care services, which position the individual as market actor. Rooting the research in the broader personalization agenda, the study looks at the limitations of the market in relation to individual purchase of private goods (e.g. home care), in the pooling of funds to purchase group services and in the provision of public goods such as building-based services. Method The article takes a multi-method approach, combining an interpretive focus on the framing of the personal budget-holder by advocates of personalization with national evaluation data, and data from a small survey of day centre workers. Results The article identifies three framings of the individual budget-holder articulated by advocates of personalization. The first is that personal budget-holders will be empowered market actors, commissioning the services they need. The second is that budget-holders will pool resources with others to purchase group services in order to broaden the range of options available to them. The third is that services which cannot be disaggregated into individual or group budgets – such as day centres – are not valued by service users. The article looks at the evaluation data on these three claims in turn. It identifies four limitations to the capacity of people to purchase care goods on an individual basis: lack of transparency in allocating budgets, complexity in managing a budget, excessive auditing of spending and lack of responsiveness from the provider market. Pooling of budgets to purchase collective services is found to be underdeveloped, and hampered by the complexity which is a broader limitation on personal budgets. Day centres are found to be closing not in response to commissioning decisions by individual budget-holders but because of decommissioning by local authorities, minimising the scope for individuals to express a preference for this type of care.     The Florida Public Service Commission (PSC) Staff recently completed an audit of fraud control policies and programs at the State`s largest regulated electric and local telephone utilities. The purpose of the audit was to examine the ability of Florida`s largest regulated utilities to deter, detect, and resolve occurrences of fraud. The Staff audited the state`s seven largest regulated electric and local telephone utilities: Florida Power Corporation, Florida Power and Light, Gulf Power Corporation, Tampa Electric Company, GTE-Florida, BellSouth Telecommunications (Southern Bell), and Sprint United/Centel. The audit scope was limited to fraudulent acts committed by employees, contractors, suppliers, or agents of the seven utilities. Information regarding the utilities` fraud control policies and programs was obtained through surveys, document requests, and interviews with managers and officers.

    In June 1995, the New York Public Service Commission (NYPSC) released a Staff investigation of Orange and Rockland Utilities, Inc. (O&R). The primary focus of the Staffs investigation was the Corporate Policy and External Affairs Department, a restricted disbursements account, the Internal Auditing Department, and O&R officer malfeasance. The Staffs` investigation uncovered widespread, alleged instances of lax internal controls, unethical and illegal actions, and lavish officer behavior. In addition, the Staff investigated O&R`s internal control and purchasing functions. The Staff proposed a series of recommendations to improve the Company`s internal control, purchasing, ethical, climate and addressed the issue of a New York ratepayer reimbursement.     Public organizations are formed in response to societal needs. They collect taxes, educate children, enforce laws and provide protection to the environment, the nation and consumers. One such organization is the New York State Public Service Commission. In 1907, legislation was passed to form the New York State Public Service Commission the first regulatory body of its kind in the United States. Its mission was to provide safe, reliable and reasonably priced electricity. Subsequently, this became the model that was implemented in every state in the nation. The past decade heralds an era of competition and a lessening of regulatory control. The telephone, natural gas and airline industries are in various stages of deregulation, and the electric industry is beginning down this path as well. In an environment such as this, are regulatory organizations necessary, and if they are, how can they organize to meet the new societal requirements? The case of the New York State Public Service Commission at this point in time offers a real time study of a regulatory body immersed in an environment that is calling for competition and an end to big government. Utilizing case studies of industries that have deregulated, or are in the process of deregulating, indicates a future societal need for regulations. This result does not lead to a conclusion that organizational change is unnecessary. This Dissertation will lay out the current organizational structure of the Public Service Commission, give an overview of the environmental signals, describe the mission/core values, and illustrate general political and employee factors that are indigenous to public service. Utilizing both classic and current organizational theory, an evaluation will be made of the Commission's need for change, their ability to change, and obstacles they may encounter.

    Regulatory agencies operate in a complex field of organizations and interest groups. The variety of behaviors that occur in the regulatory field are not satisfactorily explained by current interpretative frameworks. Regulatory agencies are at once criticized and praised. They flourish as organizational entities, yet they exhibit behavior that is often inexplicable. Notions like clientele capture, the politics of regulation, exchange theory, and institutionalized organizations do not singly explain the vagaries of regulatory behavior. A merger of these viewpoints, however, provides a foundation for a comprehensive interpretative framework. A bureau of consumer services within a state public utility commission is viewed as an institutionalized organization. From this perspective, a variety of ritual behaviors, based upon symbolic and tangible interorganizational exchanges are observed and characterized. The outcome of these exchanges is the establishment of the Bureau of Consumer Services as a legitimate regulatory agent with significant impact upon the formulation of regulatory policy in the formal proceedings of the Pennsylvania Public Utility Commission.

Summary Pharmacists who serve in the Commissioned Corps of the United States Public Health Service fill roles that are considerably different than their counterparts in the private sector. Their work takes them out from behind the counter and into the world. Pharmacy officers advance the health and safety of the Nation by their involvement in the delivery of direct patient care to medically underserved people, national security, drug vigilance, research and policy-making endeavors. PHS pharmacists fill essential public health leadership and service roles throughout the U.S. Department of Health and Human Services (HHS) and certain non-HHS federal agencies and programs. The Federal Bureau of Prisons, Health Resources and Services Administration, Food and Drug Administration, United States Coast Guard, Indian Health Service, and National Institutes of Health are among the many federal agencies where pharmacy officers are assigned.     The evolution of competition in the US natural gas industry has followed a predictable course. Wellhead deregulation stimulated pipeline restructuring, which in turn has provoked a debate over gas-service restructuring at the retail level. Over the last several years, almost all local gas distribution companies (LDCs) in the US have established stand-alone transportation service allowing industrial customers to purchase gas supplies in the open market. By all accounts, service unbundling to large retail customers has achieved significant cost savings to these customers. The current focus in the retail gas sector is on small customers-namely, small commercial and residential customers. To many observers, service unbundling to small customers, especially residential customers, is not as clear cut in terms of yielding economic benefits as it was for large customers. For example, they question whether residential customers or their agents can procure gas supplies and interstate pipeline services at a lower cost than an LDC. They also argue that the transaction cost for small customers, in terms of per-unit of gas purchased, may be much greater than for large customers. Finally, they believe the high cost of unreliable service to small customers may preclude reliance on market forces and contracts, to assure these customers the high level of reliable service that they demand. Comprehensive service unbundling with the correct regulatory rules in place should further enhance competition in the natural gas industry. If past trends in the natural gas industry continue, service unbundling will ultimately be available to all retail customers. This article examines the many regulatory-policy questions relating to the unbundling of services to small retail gas customers. It argues that widespread service unbundling is an inherent feature of a competitive natural gas industry and will likely benefit gas customers and society at large. 61 refs.

In this survey, we approached all mental health and neuropsychiatric service commissioners within London to map current funding and commissioning arrangements, and explored perceived barriers to neuropsychiatric service commissioning. Results 83% of commissioners within London responded. There was significant variability between neuropsychiatric services commissioned through the mental health stream. Contracting arrangements were variable. Lack of earmarked fund for neuropsychiatry and disjointed funding stream for such services were identified by commissioners as a barrier, as was the critical mass of neuropsychiatric cases. Clinical implications Neuropsychiatric service development continues to be hindered by lack of clear commissioning process.     As the fifth national bioethics commission has concluded its work and a sixth is currently underway, it is time to step back and consider appropriate measures of success. This paper argues that standard measures of commissions' influence fail to fully assess their role as public forums. From the perspective of democratic theory, a critical dimension of this role is public engagement: the ability of a commission to address the concerns of the general public, to learn how average citizens resolve moral issues in healthcare, and to monitor public opinion on the topics addressed in the commission. Such a public forum role is supported by the critical literature within bioethics, which has deemed some commissions successful, supported more generally by the history of bioethics as a reform discourse that has brought socially important values into the medical domain, and supported more generally still by the example of the great social issues commissions of the 1960s.     Australia has a complex history of providing public dental services to its communities. From the early days of Colonial settlement, the provision of dental care to the Australian public has largely been driven and influenced by organized groups and associations of dentists. The Constitution of Australia, under Section 51 xxiii A, allows for the Commonwealth to provide for medical and dental services. Unlike the United Kingdom, however, dental services have not been embedded into a universal national health service agenda. In 1974, that the Australian Government through the Australian School Dental Program provided the first funding and national direction for public dental services - and that, limited to children. The Commonwealth Dental Health Program 1993-1997 was the second national endeavor to provide public dental services, this time to financially disadvantaged adults. Since that time, public dental service responsibility has been shuttled between States/Territories and the Commonwealth. A new paradigm for public dental services in Australia requires strong Commonwealth leadership, as well as the commitment of State and Territories and the organized dental profession. The National Health and Hospitals Reform Commission provided the most recent scenario for a radical change in mission. This paper canvases the competing roles of strategic, functional, and structural issues in relationship to social network and policy issues, which must be recognized if Australians truly seek to reform public dental services.   9. A summary of the California Public Utilities Commission`s two competing electric utility restructuring proposals

    In May 1995, the California Public Utilities Commission (CPUC) released two proposals for restructuring the state`s electric power industry. The two proposals follow more than a year of testimony and public comment after the CPUC issued the ``Blue Book`` (CPUC 1994a) on April 20, 1994, which called for retail wheeling to be phased in to all customers over 5 years. The majority proposal, supported by three of the four CPUC commissioners (one seat was vacant when the proposals were released), calls for creating a central pool, or ``poolco``; setting electric prices to reflect true costs of service, or ``real-time pricing``; and allowing parties to negotiate ``contracts for differences`` between the pool price and the contract price. The minority proposal, sponsored by Commissioner Jesse Knight, calls for retail wheeling, or ``direct access,`` and for utilities to divest or spin off their generating assets. This paper presents a summary of the major provisions of the two CPUC proposals and the possible implications and issues associated with each. It is aimed at researchers who may be aware that various efforts to restructure the electric power industry are under way and want to known more about California`s proposals, as well as those who want to known the implications of certain restructuring proposals for renewable energy technologies. Presented at the end of the paper is a summary of alternative proposals promoted by various stakeholder in response to the two CPUC proposals.

    This article is a critical reflection on the role of the nurse in commissioning a service within the primary care setting. It will use the fictitious example of commissioning a nurse-led crisis prevention service in the London borough of Lambeth as an exemplar to highlight the difficulties surrounding the commissioning process. In placing particular focus on the prevalence of smoking, it is suggested that designing services based around tackling 'clusters' of unhealthy risk factors such as smoking, diet and excessive alcohol consumption may be a more holistic approach to delivering better healthcare outcomes for more socioeconomically deprived populations as opposed to previous national siloed attempts (Buck and Forsini 2012;1). It will argue that despite multifaceted and evolving roles, community nurses are ideally placed to recognise compounding risk factors detrimental to health as they work at the interface between the individual and their environment. This awareness can be used to positively impact on the commissioning process but only if greater attention is paid towards enhancing leadership skills throughout nursing, and the rhetoric of effective collaboration across agencies is translated into practice (Ham et al, 2012; NHS Commissioning Board (NHS CB), 2012), NHS Alliance, 2011).     The IAU Division C Commission 55, Communicating Astronomy with the Public, played an active role in Union affairs within Division C, Education, Outreach and Heritage. The International Astronomical Union (IAU) vested considerable responsibility for its public outreach efforts in Commission 55 (C55), Communicating Astronomy with the Public. This article briefly recounts the origin and history of C55 over the past decade, and describes the work of C55 until it became Division C Commission C.C2 in 2015. As stated on our website, http://www.communicatingastronomy.org, C55 was founded on the principle that ``it is the responsibility of every practising astronomer to play some role in explaining the interest and value of science to our real employers, the taxpayers of the world.'' While this was true a decade ago, when the Working Group that eventually became C55 first took shape, it is even more true today, when funding for the astronomical sciences (and science more generally) is under threat on nearly every continent.

    The National Aeronautics and Space Administration (NASA) Technology Utilization (TU) Division of the Office of Commercial Programs has been quite successful in directing the transfer to technology into the public sector. NASA developments of particular interest have been those in the areas of aerodynamics and aviation transport, safety, sensors, electronics and computing, and satellites and remote sensing. NASA technology has helped law enforcement, firefighting, public transportation, education, search and rescue, and practically every other sector of activity serving the U.S. public. NASA works closely with public service agencies and associations, especially those serving local needs of citizens, to expedite technology transfer benefits. A number of examples exist to demonstrate the technology transfer method and opportunities of NASA spinoffs to public service.

    A good fraction of the Commission 55 (C55) Organizing Committee met in Beijing in August at the XXVIII IAU General Assembly, where C55 organized Special Session 14 (SpS14) entitled ``Communicating Astronomy with the Public for Scientists.'' During our C55 business meeting, and again during an impromptu gathering a few days later, we discussed changes in the IAU's organizational and programmatic structure and how these changes might affect C55.     Public service programs using NASA's ATS-6 and CTS satellites are discussed. Examples include the ATS-6 Health and Education Telecommunications experimental program and the use of CTS to enable students in one university to take courses presented at another distant university. Possible applications of satellite communication systems to several areas of public service are described, and economic and political obstacles hindering the implementation of these programs are considered. It is suggested that a federally sponsored program demonstrating the utility of satellites accomodating a large number of small terminals is needed to encourage commercial satellite operations.

    The commissioning and provision of healthcare, including mental health services, must be consistent with ethical principles - which can be summarised as being "fair", irrespective of the method chosen to deliver care. They must also provide value to both patients and society in general. Value may be defined as the ratio of patient health outcomes to the cost of service across the whole care pathway. Particularly in difficult times, it is essential to keep an open mind as to how this might be best achieved. National and regional policies will necessarily vary as they reflect diverse local histories, cultures, needs and preferences. As systems of commissioning and delivering mental health care vary from country to country, there is the opportunity to learn from others. In the future international comparisons may help identify policies and systems that can work across nations and regions. However a persistent problem is the lack of clear evidence over cost and quality delivered by different local or national models. The best informed economists, when asked about the international evidence do not provide clear answers, stating that it depends how you measure cost and quality, the national governance model and the level of resources. The UK has a centrally managed system funded by general taxation, known as the National Health Service (NHS). Since 2010, the UK's new Coalition* government has responded by further reforming the system of purchasing and providing NHS services - aiming to strengthen choice and competition between providers on the basis of quality and outcomes as well as price. Although the present coalition government's intention is to maintain a tax-funded system, free at the point of delivery, introducing market-style purchasing and provider-side reforms to encompass all of these bring new risks, whilst not pursuing reforms of a system in crisis is also seen to carry risks.     Accountable Care Organizations (ACOs) in the United States of America (USA) and Clinical Commissioning Groups (CCGs) in the United Kingdom (UK) are newly proposed cross-organisational structures in health services both tasked with a role which includes improving public health. Although there are very significant differences between the UK and USA health systems, there appears to be some similar confusion as to how ACOs and CCGs will regard and address public or population health. This short perspective article gives an overview of ACOs in the USA and CCGs in the UK, with the underlying context of possible public health functions. It concludes by considering the challenges facing both countries and highlighting the opportunity for shared learning.     Police custody healthcare services for detainees in the UK are most commonly outsourced to independent healthcare providers who employ custody nurses and forensic physicians to deliver forensic healthcare services. A pilot was introduced in 2008 by the Department of Health to explore the efficacy of commissioning custody healthcare via the NHS, in the wake of the 2005-2006 shift of prison healthcare to the NHS. The objective was to improve quality and accountability through NHS commissioning and the introduction of NHS governance to the management and delivery of custody healthcare. This article discusses key themes that arose from the project evaluation, which focused on the commissioning relationship between the police, the NHS commissioner and the private healthcare provider. The evaluation observed an evolving relationship between the police, the local NHS and the front-line nurses, which was complicated by the quite distinctive professional values and ideologies operating, with their contrasting organisational imperatives and discordant values and principles. A key challenge for commissioners is to develop synergy between operational and strategically located stakeholders so that they can work effectively towards common goals. Government policy appears to remain focused on creating safe, supportive and humane custody environments that balance criminal justice and health imperatives and support the rights and needs of detainees, victims, professionals and the public. This remains an ambitious agenda and presents a major challenge for new criminal justice health partnerships. PMID:23910844

    Large blackouts, such as the August 14-15, 2003 blackout in the northeasternUnited States and Canada, focus attention on the importance of reliable electric service. As public and private efforts are undertaken to improve reliability and prevent power interruptions, it is appropriate to assess their effectiveness. Measures of reliability, such as the frequency and duration of power interruptions, have been reported by electric utilities to state public utility commissions for many years. This study examines current state and utility practices for collecting and reporting electricity reliability information and discusses challenges that arise in assessing reliability because of differences among these practices. The study is based primarily on reliability information for 2006 reported by 123 utilities to 37 state public utility commissions.

    Regulators operate in a constantly changing environment characterized by new technologies, increased competition in regulated fields, and heightened environmental awareness. In such an environment, they must maintain a balance between the customers` interest in low rates for safe and reliable utility service, and the need to maintain solvent utilities that have the ability to attract capital at a reasonable cost. Concentration on either end of that balance by a regulatory agency during any particular period brings almost inevitably, charges of unfairness by champions representing the other side of the balance. If, for example, a Public Utilities Commission (PUC) sees the need to concentrate on the financial health of the utilities under its jurisdiction, consumers will likely feel the sting of rate increases. In the author`s view, Capture Theory, a theory of regulatory behavior which held sway for decades, resulted from scholars viewing regulatory agencies during the part of a cycle of regulation, when the concentration was on that side of the issue. But even while the various forms of Capture Theory were ascendant in the mid-1970`s, events in the field of public utility regulation were serving to refute them all. {open_quotes}Instead of having the happy task of presiding over a process that seemed to bestow benefits upon everyone, regulators in the 1970`s were faced with the unpleasant duty of allocating misery among rival and intensely vocal groups.{close_quotes}

    Conscious sedation is an integral part of modern day dental care and should be delivered through a high quality, effective and evidence-based approach. Commissioning of NHS dental services in England is currently under review by NHS England and the National Dental Commissioning Group. This group has identified the management of vulnerable people including anxious patients, as one of its priorities. The Society for the Advancement of Anaesthesia in Dentistry (SAAD) believes this provides an opportunity to influence the commissioning of NHS conscious sedation services. With this aim in mind,"Guidance for Commissioning NHS England Dental Conscious Sedation Services: A Framework Tool" was developed. This guidance proposes a common approach to the organisation of NHS dental conscious sedation services in England, advocating the provision of Tier 1 and Tier 2 services in all regions. Its ethos is a"hub and spoke" model of service delivery with patient assessment delivered by experienced and well trained dental sedationists at its core. In line with the recent Francis Report fundamental standards for all aspects of dental conscious sedation practice are outlined, supported by a robust and predictable quality assurance process.     The further division of responsibilities between commissioners and providers in England will have far-reaching consequences and opportunities for developing and enhancing health improvement. Commissioners will have the opportunity to craft local solutions to local issues. To be effective, these local responses need to tackle the core determinants of health and to build the social capital that is at the heart of all communities that enjoy high standards of health. This paper argues that the new arrangements mark an evolution of the Beveridge model of healthcare (centralized, top down and professionally prescribed) to a post-Beveridge model characterized by it being decentralized, localized and utilizing professional skills in the pursuit of client and community satisfaction and engagement rather than a narrowly defined professional perspective. This paper indicates some of the key conceptual changes commissioners need to employ to take advantage of the emerging opportunities. It is argued that the new arrangements will only be fully effective if commissioners of health improvement programmes ensure they factor in health promotion and social marketing expertise, both in the strategic and operational phases of commissioning.     A lawyer who was assistant director of the President's Commission for the Study of Ethical Problems in Medicine and Biomedical and Behavioral Research describes the difficulties he sees in participation by philosophers in the making of public policy. He attributes the Commission's considerable success in influencing public policy to its consensus building approach. However, in its analysis of the appropriate governmental response to compensation of injured research subjects, the Commission produced only a "useless" report calling for further study. The problem was that the force of the simple argument for compensation based on justice was undercut by an academic libertarian argument. Technical questions of program design are refractory to philosophical analysis, and philosophers are not receptive to pragmatic compromise. Accordingly, philosophers should provide a moral critique of political compromises, rather than play a direct role in policy making. PMID:11658835

  13. Challenges of commissioning and contracting for integrated care in the National Health Service (NHS) in England.

    For many years there has been a separation between purchasing and provision of services in the English National Health Service (NHS). Many studies report that this commissioning function has been weak: purchasers have had little impact or power in negotiations with large acute providers, and have had limited strategic control over the delivery of care. Nevertheless, commissioning has become increasingly embedded in the NHS structure since the arrival of Clinical Commissioning Groups (CCGs) in 2012. Recently, some of these CCGs have focused on how they can contract and commission in different ways to stimulate greater collaboration across providers. This paper examines experiences of commissioning and contracting for integrated care in the English NHS, based on a series of national-level interviews and case studies of five health economies that are implementing novel contracting models. The cases illustrated here demonstrate early experiments to drive innovation through contracting in the NHS that have largely relied on the vision of individual teams or leaders, in combination with external legal, procurement and actuarial support. It is unlikely that this approach will be sustainable or replicable across the country or internationally, despite the best intentions of commissioners. Designing and operating novel contractual approaches will require considerable determination, alongside advanced skills in procurement, contract management and commissioning. The cost of developing new contractual approaches is high, and as the process is difficult and resource-intensive, it is likely that dedicated teams or programs will be required to drive significant improvement.   8. Promises and perils of public deliberation: contrasting two national bioethics commissions on embryonic stem cell research.

    National bioethics commissions have struggled to develop ethically warranted methods for conducting their deliberations. The National Bioethics Advisory Commission in its report on stem cell research adopted an approach to public deliberation indebted to Rawls in that it sought common ground consistent with shared values and beliefs at the foundation of a well-ordered democracy. In contrast, although the research cloning and stem cell reports of the President's Council on Bioethics reveal that it broached two different methods of public deliberation--balancing goods and following an overarching moral principle--it adopted neither. Thereupon its primer mover, Leon Kass, influenced particularly by the approach of Leo Strauss, sought to develop a method of public deliberation guided by tradition and practical wisdom. When this failed, the Council fell back on a method that took account of shared fundamental values of a free democracy--a method remarkable akin to that employed by the National Bioethics Advisory Commission. Respect for diverse reasonable conceptions of the good in a democratic polity requires national bioethics commissions to seek and incorporate that which is valuable in opposing positions.
