    The first of several state-of-the-art sampling instruments to monitor acid fog in the South Coast Air Basin on an on-going basis has been in stalled in Rubidoux by the South Coast Air Quality Management District. The automated equipment, called the Caltech Active Strand Collector (CASC), is part of a long-term acid fog monitoring program developed by AQMD. The collecting process involves drawing a fog-laden air sample into the collector where fog droplets strike a series of teflon strands and run down to a collection trough. The sample is then sent to AQMD's laboratory to determine acidity and chemical composition. The monitoring equipment will be moved to Pomona later this winter, and to Crestline in the spring. Following this initial evaluation period, additional CASC units will be sited in the region.

    In 2005, Naturally Occurring Asbestos (NOA) in El Dorado County made headlines with the discovery significant amounts of tremolite/actinolite asbestos in areas where residents had built, or were in the process of building, homes and residences. The El Dorado Air Quality Management District has been involved in all aspects of dealing with NOA from the very beginning of its discovery, from overseeing the rehabilitation of school sites to expanding and rewriting fugitive dust rules at construction sites. A discussion of best management practices which have been developed will be given, as well as how the El Dorado Air Quality Management District has worked to educate members of the public, as well as workers in the field, about NOA to aid in maintaining the health and safety of the public.

    A tension exists between objectives of health policy makers to achieve high quality standards of care on one hand, and district multi-crisis reality in sub-Saharan Africa on the other hand where sheer survival of the (public) health system is questioned. The collapsing health services as well as the deteriorating living conditions affect the health status of the population and contribute to an increase in (health) inequalities both nationally and internationally. Constraints and some examples of achievements in district health management in two districts in Northern Province, Zambia, are presented. A strong focus on community-based health care, partnership with communities and accelerated health-system support via strengthening of on-site supervision is advocated. Decentralization and self-reliance are potential tools in flexible crisis management, but require continuity in human resource development and appropriate 'care for the carers'. In order to increase operational efficiency, the need is expressed to conceptualize a practical approach of 'minimum primary health care'. PMID:7747327

    The purpose of the research project was to advance the concept of real-time water quality management in the San Joaquin Basin by developing an application to drainage of seasonal wetlands in the Grassland Water District. Real-time water quality management is defined as the coordination of reservoir releases, return flows and river diversions to improve water quality conditions in the San Joaquin River and ensure compliance with State water quality objectives. Real-time water quality management is achieved through information exchange and cooperation between shakeholders who contribute or withdraw flow and salt load to or from the San Joaquin River. This project complements a larger scale project that was undertaken by members of the Water Quality Subcommittee of the San Joaquin River Management Program (SJRMP) and which produced forecasts of flow, salt load and San Joaquin River assimilative capacity between 1999 and 2003. These forecasts can help those entities exporting salt load to the River to develop salt load targets as a mechanism for improving compliance with salinity objectives. The mass balance model developed by this project is the decision support tool that helps to establish these salt load targets. A second important outcome of this project was the development and application of a methodology for assessing potential impacts of real-time wetland salinity management. Drawdown schedules are typically tied to weather conditions and are optimized in traditional practices to maximize food sources for over-wintering wildfowl as well as providing a biological control (through germination temperature) of undesirable weeds that compete with the more proteinaceous moist soil plants such as swamp timothy, watergrass and smartweed. This methodology combines high resolution remote sensing, ground-truthing vegetation surveys using established survey protocols and soil salinity mapping using rapid, automated electromagnetic sensor technology.     As the city is urbanized, its landscape is getting more complex due to the construction of high-rise buildings. The smaller scale wind-field in an urban district may change frequently due to the complex terrain, the diverse landuse, and high-rise buildings. It also leads to dynamic changes of air pollution in that area. The conventional urban scale air quality management system, however, is too coarse to effectively manage such a small area. In this study, we set up a micro-scale air quality management testbed near Konkuk University, Seoul, Korea. A ubiquities sensor monitoring network, high resolution emission database, and CFD-based air quality modeling system were developed, and then applied to the testbed. A sensor data management system using wireless technology and multi-modal scientific visualization module were combined in support of the management system. The sensor based monitoring system shows reasonably good performance for wind speed, temperature, and carbon dioxide from inter-comparison study against conventional large format analyzers. The sensor data have been successfully collected using a wireless sensor data collection network during a 6months operation period from July, 2009. The fire pollution event simulation using the CFD model reveals the effect of high rise buildings in the testbed.

    In this study, an inexact double-sided fuzzy-random-chance-constrained programming (IDSFRCCP) model was developed for supporting air quality management of the Nanshan District of Shenzhen, China, under uncertainty. IDSFRCCP is an integrated model incorporating interval linear programming and double-sided fuzzy-random-chance-constrained programming models. It can express uncertain information as both fuzzy random variables and discrete intervals. The proposed model was solved based on the stochastic and fuzzy chance-constrained programming techniques and an interactive two-step algorithm. The air quality management system of Nanshan District, including one pollutant, six emission sources, six treatment technologies and four receptor sites, was used to demonstrate the applicability of the proposed method. The results indicated that the IDSFRCCP was capable of helping decision makers to analyse trade-offs between system cost and risk of constraint violation. The mid-range solutions tending to lower bounds with moderate αh and qi values were recommended as decision alternatives owing to their robust characteristics.

    Hydrologic, physicochemical, and aquatic community data were collected and compiled by the U.S. Geological Survey for selected springs within the St. Johns River Water Management District from January 2004 to October 2007. Nine springs were included in this study: Alexander, Apopka, Bugg, De Leon, Gemini, Green, Rock, Silver Glen, and Wekiwa. Urban lands increased in Alexander, Apopka, De Leon, Gemini, Green, and Wekiwa springsheds between 1973 and 2004, accompanied by a loss of forested and/or agricultural lands in most springsheds. Forested cover increased and open surface waters and wetlands decreased in the Bugg and Rock springsheds. Although rainfall did not change significantly over time in each springshed, spring discharge decreased significantly in De Leon, Fern Hammock, Rock, Silver, and Wekiwa Springs. Nitrate concentrations increased significantly with time in Apopka, Fern Hammock, Gemini Springs run, and Juniper Springs, and decreased significantly in Alexander Spring, Bugg Spring run, Rock Springs, and Wekiwa Springs. Phosphorus increased significantly with time in Juniper Springs and decreased significantly in Apopka, De Leon, Rock, Silver Glen, and Wekiwa Springs. Benthic macroinvertebrate communities ranged from relatively low diversity assemblages (Green Spring) to assemblages with high taxonomic richness, diversity, and dominance (Rock and De Leon Springs). Shannon-Wiener diversity index averages among samples pooled by spring were lowest for Apopka Spring and greatest for Rock, Bugg, and Silver Glen Springs. Mean Stream Condition Index for pooled samples per spring was lowest for De Leon and Gemini Springs and highest for Rock and Wekiwa Springs. Mean percentages of very tolerant taxa were lowest for Alexander Spring and highest for Bugg and Green Springs. Fish community richness was lowest for Green Spring, and greatest for Alexander Spring run and Silver Glen Springs. Forty five fish species representing 35 genera and 23 families were collected

    All well-known quality management (QM) models in health care also address issues of the quality of leadership, even if they do so with varying intensity. Skills in managing organisations are a basically important quality in a top manager, but skills in leading people have been revealed as being most relevant. All the manuals of the most common quality management systems specify 'To-Do' lists and tasks for the top management--EFQM and KTQ/proCum Cert have definitively linked them with quality management. Quality needs to be assessed by the degree to which management goals are achieved. However, there is no easy approach to measure leadership abilities and explain it to the public. Since this is a real challenge and very difficult to achieve it is not a central focus of the various QM manuals. PMID:19545082

    Revenues for public education are declining and school budgets are being evaluated for ways to cut waste. The maintenance and operations budget often falls victim to the budget ax as maintenance on equipment and buildings is deferred. It may be that as a result of the deferred maintenance, school districts are actually spending more on maintenance and operations and energy than before. This study sought to determine the status of energy use in Louisiana's 66 public school districts and to determine the need for an energy management education program in those districts. Data for the 1991-1992 school year were collected using a mailed questionnaire from 55 of the 66 public school distrincts. Intensive non-response follow-up procedures were instituted to obtain a complete profile of the existing energy expenditure situation in the state's public school districts. Per square foot expenditures for energy were calculated for the 55 school districts that responded. A per student expenditure for energy was calculated for all 66 school districts. Per square foot and per student expenditures for energy and for maintenance and operations were compared to the state average and to the southern region average. The relationship between per student and per square foot expenditures was determined. The relationship between maintenance and operations and energy was also examined. School districts within the state were prioritized in order of need for an energy management program. Respondents rated 14 energy-related concepts as to their perceived importance for inclusion in an educational program on energy management for school maintenance and operations personnel. Findings indicate that a wide variation of per student and per square foot expenditures for energy and maintenance and operations exists within the state. No relationship was found between energy expenditures and maintenance and operations expenditures.

It is essential that district nurses and community matrons discharge their duty of care to patients in care homes by supporting effective medication management that meets the individual needs of those patients.     An evaluation was made of an existing monitoring program in the Panoche Water District for 1986-93. The Panoche Water District is an agricultural area located in the western San Joaquin Valley of California. Because irrigation drainage from this area has high concentrations of dissolved solids and selenium, management strategies have been developed to improve the quality of drainwater discharge. The purpose of the Panoche Water District's monitoring program is to assess the effects of water- and land-use practices on local ground water and drain flow from the district. Drainflow from the district consists of the discharge from 50 separate on-farm underground tile-drainage systems. The Panoche Water District maintains information on water deliveries, planned and actual crop types, and planned and actual acreages planted each year. In addition, the water district monitors ground-water and drainage-system discharges using a variety of data-collection methods. A total of 62 observation well sites are used to monitor ground-water level and quality. A total of 42 sites were monitored for drainflow quantity, and drain flow quality samples were collected from the outlets of each of the 50 drainage systems. However, these data were collected inconsistently and (or) intermittently during the period studied. All data obtained from the water district were compiled and stored in a geographic information system database. Water delivered for irrigation by the Panoche Water District is a mix of imported water and local ground water pumped directly into delivery canals. Although delivered water is a mix, information on the proportion of water from the two sources is not reported. Also, individual growers pump directly to their crops unknown quantities of ground water, the total of which could be greater than 60 percent of total applications during years when water district deliveries are greatly reduced (for example, the years during and following a drought).     The South Adams County Water and Sanitation District`s most productive shallow alluvial wells are contaminated with a number of volatile organic chemicals. In addition, previous and ongoing studies by various agencies have detected several nonvolatile organic contaminants clearly associated with Rocky Mountain Arsenal (RMA) operations in the northern portion of the existing District area and throughout the western half of the area north and west of RMA. This area is ultimately planned as an addition to the District`s present service area. The District retained the services of James M. Montgomery, Consulting Engineers, Inc. (JMM) to evaluate treatment alternatives for removal of organics contaminants and softening. The objectives of the study are outlined in the District`s request for proposals dated October 11, 1985. In general, the objectives are: to establish, through an analysis of cost and availability, potential sources of supply for the current and projected District water demands; based on requirements of the Colorado Department of Health and the Environmental Protection Agency, develop treated water quality goals; and select the most feasible treatment for organic contaminant removal with consideration of cost, constructability and reliability in meeting treated water quality goals.

    The U.S. Geological Survey (USGS), Water Resources Division (WRD), requires that quality-assurance/quality-control (QA/QC) activities be included in any sampling and analysis program. Operational QA/QC procedures address local needs while incorporating national policies. Therefore, specific technical policies were established for all activities associated with water-quality project being done by the Ohio District. The policies described in this report provide Ohio District personnel, cooperating agencies, and others with a reference manual on QA/QC procedures that are followed in collecitng and analyzing water-quality samples and reporting water-quality information in the Ohio District. The project chief, project support staff, District Water-Quality Specialist, and District Laboratory Coordinator are all involved in planning and implementing QA/QC activities at the district level. The District Chief and other district-level managers provide oversight, and the Regional Water-Quality Specialist, Office of Water Quality (USGS headquarters), and the Branch of Quality Systems within the Office of Water Quality create national QA/QC polices and provide assistance to District personnel. In the literature, the quality of all measurement data is expressed in terms of precision, variability, bias, accuracy, completeness, representativeness, and comparability. In the Ohio District, bias and variability will be used to describe quality-control data generated from samples in the field and laboratory. Each project chief must plan for implementation and financing of QA/QC activities necessary to achieve data-quality objectives. At least 15 percent of the total project effort must be directed toward QA/QC activities. Of this total, 5-10 percent will be used for collection and analysis of quality-control samples.     In 1994, a national quality assurance programme was established in Uganda to strengthen district-level management of primary health care services. Within 18 months both objective and subjective improvements in the quality of services had been observed. In the examples documented here, there was a major reduction in maternal mortality among pregnant women referred to Jinja District Hospital, a reduction in waiting times and increased patient satisfaction at Masaka District Hospital, and a marked reduction in reported cases of measles in Arua District. Beyond these quantitative improvements, increased morale of district health team members, improved satisfaction among patients, and greater involvement of local government in the decisions of district health committees have been observed. At the central level, the increased coordination of activities has led to new guidelines for financial management and the procurement of supplies. District quality management workshops followed up by regular support visits from the Ministry of Health headquarters have led to a greater understanding by central staff of the issues faced at the district level. The quality assurance programme has also fostered improved coordination among national disease-control programmes. Difficulties encountered at the central level have included delays in carrying out district support visits and the failure to provide appropriate support. At the district level, some health teams tackled problems over which they had little control or which were overly complex; others lacked the management capacity for problem solving. PMID:9185368

    Background South Africa is currently focusing strongly on human resource development. The purpose of this study was to describe and compare the quality of nursing service and care in three health districts in the KwaZulu Natal Province. To identify deficiencies which could be addressed by education and training, it might be useful to measure the quality of care given by nurses. Methods From March to August 2002 a survey was done in six hospitals and six clinics in three health districts of the KwaZulu-Natal province of South Africa. Five different aspects of care was evaluated; hand-over from one nursing shift to another, implementation of universal precautions, patient satisfaction, nursing records, management of chronic illnesses. All these aspects were evaluated using checklists based on record reviews or direct observation, except for patient satisfaction, which was evaluated by questionnaires. Results The average scores on the different aspects varied from 11% (for nursing records) to 73% (for management of chronic diseases). Specific problems became evident. In one district three out of four hand-overs between shifts of nurses scored less than 50%. In all three districts the use of protective gear scored low (43%). While the average score for management of chronic illnesses were high at 73%, the blood pressures of only 23% was within the target range, and the blood sugar of only 38% of patients were controlled. Patient satisfaction averaged 72% across the three districts. Conclusion The quality of care measurements identified specific training needs, but other management strategies are probably also indicated. PMID:15028123

    Biomedical investigators require high quality human tissue to support their research; thus, an important aspect of the provision of tissues by biorepositories is the assurance of high quality and consistency of processing specimens. This is best accomplished by a quality management system (QMS). This article describes the basis of a QMS program designed to aid biorepositories that want to improve their operations. In 1983, the UAB Tissue Collection and Biobanking Facility (TCBF) introduced a QMS program focused on providing solid tissues to support a wide range of research; this QMS included a quality control examination of the specific specimens provided for research. Similarly, the Division of Laboratory Sciences at the Centers for Disease Control and Prevention (CDC) introduced a QMS program for their laboratory analyses, focused primarily on bodily fluids. The authors of this article bring together the experience of the QMS programs at these two sites to facilitate the development or improvement of quality management systems of a wide range of biorepositories.     Biomedical investigators require high quality human tissue to support their research; thus, an important aspect of the provision of tissues by biorepositories is the assurance of high quality and consistency of processing specimens. This is best accomplished by a quality management system (QMS). This article describes the basis of a QMS program designed to aid biorepositories that want to improve their operations. In 1983, the UAB Tissue Collection and Biobanking Facility (TCBF) introduced a QMS program focused on providing solid tissues to support a wide range of research; this QMS included a quality control examination of the specific specimens provided for research. Similarly, the Division of Laboratory Sciences at the Centers for Disease Control and Prevention (CDC) introduced a QMS program for their laboratory analyses, focused primarily on bodily fluids. The authors of this article bring together the experience of the QMS programs at these two sites to facilitate the development or improvement of quality management systems of a wide range of biorepositories.   11. Managing complex palliative wounds: an interactive educational approach for district nurses.

    Complex palliative wounds, although uncommon, are extremely distressing for patients because of their associated symptoms. Managing these symptoms presents significant challenges to clinicians. As the majority of these patients will be managed at home, district nurses will be the main providers of care. The quality of the literature in this field is limited due to the small number of patients presenting with these wounds and the difficulties associated with researching issues within palliative care. However, the literature available identifies that community nurses would value greater education in managing these wounds because the accepted wound healing theories and management strategies do not apply. This underpins the rationale for developing an educational resource to provide district nurses with current, evidence-based information to support their wound-management decisions in caring for this patient group.   12. An assessment of the quality of care for children in eighteen randomly selected district and sub-district hospitals in Bangladesh

    Background Quality hospital care is important in ensuring that the needs of severely ill children are met to avert child mortality. However, the quality of hospital care for children in developing countries has often been found poor. As the first step of a country road map for improving hospital care for children, we assessed the baseline situation with respect to the quality of care provided to children under-five years age in district and sub-district level hospitals in Bangladesh. Methods Using adapted World Health Organization (WHO) hospital assessment tools and standards, an assessment of 18 randomly selected district (n=6) and sub-district (n=12) hospitals was undertaken. Teams of trained assessors used direct case observation, record review, interviews, and Management Information System (MIS) data to assess the quality of clinical case management and monitoring; infrastructure, processes and hospital administration; essential hospital and laboratory supports, drugs and equipment. Results Findings demonstrate that the overall quality of care provided in these hospitals was poor. No hospital had a functioning triage system to prioritise those children most in need of immediate care. Laboratory supports and essential equipment were deficient. Only one hospital had all of the essential drugs for paediatric care. Less than a third of hospitals had a back-up power supply, and just under half had functioning arrangements for safe-drinking water. Clinical case management was found to be sub-optimal for prevalent illnesses, as was the quality of neonatal care. Conclusion Action is needed to improve the quality of paediatric care in hospital settings in Bangladesh, with a particular need to invest in improving newborn care. PMID:23268650

    Objective To describe healthcare worker (HCW)-identified system-based bottlenecks and the value of local engagement in designing strategies to improve referral processes related to emergency obstetric care in rural Ghana. Design Qualitative study using semistructured interviews of participants to obtain provider narratives. Setting Referral systems in obstetrics in Assin North Municipal Assembly, a rural district in Ghana. This included one district hospital, six health centres and four local health posts. This work was embedded in an ongoing quality improvement project in the district addressing barriers to existing referral protocols to lessen delays. Participants 18 HCWs (8 midwives, 4 community health officers, 3 medical assistants, 2 emergency room nurses, 1 doctor) at different facility levels within the district. Results We identified important gaps in referral processes in Assin North, with the most commonly noted including recognising danger signs, alerting receiving units, accompanying critically ill patients, documenting referral cases and giving and obtaining feedback on referred cases. Main root causes identified by providers were in four domains: (1) transportation, (2) communication, (3) clinical skills and management and (4) standards of care and monitoring, and suggested interventions that target these barriers. Mapping these challenges allowed for better understanding of next steps for developing comprehensive, evidence-based solutions to identified referral gaps within the district. Conclusions Providers are an important source of information on local referral delays and in the development of approaches to improvement responsive to these gaps. Better engagement of HCWs can help to identify and evaluate high-impact holistic interventions to address faulty referral systems which result in poor maternal outcomes in resource-poor settings.   4. What Do District Health Managers in Ghana Use Their Working Time for? A Case Study of Three Districts

    Background Ineffective district health management potentially impacts on health system performance and service delivery. However, little is known about district health managing practices and time allocation in resource-constrained health systems. Therefore, a time use study was conducted in order to understand current time use practices of district health managers in Ghana. Methods All 21 district health managers working in three districts of the Eastern Region were included in the study and followed for a period of three months. Daily retrospective interviews about their time use were conducted, covering 1182 person-days of observation. Total time use of the sample population was assessed as well as time use stratified by managerial position. Differences of time use over time were also evaluated. Results District health managers used most of their working time for data management (16.6%), attending workshops (12.3%), financial management (8.7%), training of staff (7.1%), drug and supply management (5.0%), and travelling (9.6%). The study found significant variations of time use across the managerial cadres as well as high weekly variations of time use impulsed mainly by a national vertical program. Conclusions District health managers in Ghana use substantial amounts of their working time in only few activities and vertical programs greatly influence their time use. Our findings suggest that efficiency gains are possible for district health managers. However, these are unlikely to be achieved without improvements within the general health system, as inefficiencies seem to be largely caused by external factors. PMID:26068907

    Background Local health system managers in low- and middle-income countries have the responsibility to set health priorities and allocate resources accordingly. Although tools exist to aid this process, they are not widely applied for various reasons including non-availability, poor knowledge of the tools, and poor adaptability into the local context. In Uganda, delivery of basic services is devolved to the District Local Governments through the District Health Teams (DHTs). The Community and District Empowerment for Scale-up (CODES) project aims to provide a set of management tools that aid contextualised priority setting, fund allocation, and problem-solving in a systematic way to improve effective coverage and quality of child survival interventions. Design Although the various tools have previously been used at the national level, the project aims to combine them in an integral way for implementation at the district level. These tools include Lot Quality Assurance Sampling (LQAS) surveys to generate local evidence, Bottleneck analysis and Causal analysis as analytical tools, Continuous Quality Improvement, and Community Dialogues based on Citizen Report Cards and U reports. The tools enable identification of gaps, prioritisation of possible solutions, and allocation of resources accordingly. This paper presents some of the tools used by the project in five districts in Uganda during the proof-of-concept phase of the project. Results All five districts were trained and participated in LQAS surveys and readily adopted the tools for priority setting and resource allocation. All districts developed health operational work plans, which were based on the evidence and each of the districts implemented more than three of the priority activities which were included in their work plans.     Over period of time, the water usage and management is under stress for various reasons including pollution in both surface and subsurface. The groundwater quality decreases due to the solid waste from urban and industrial nodes, rapid use of insecticides and pesticides in agricultural practices. In this study, ground water quality maps for Rupnagar district of Punjab has been prepared using geospatial interpolation technique through Inverse Distance Weighted (IDW) approach. IDW technique has been used for major ground water quality parameters observed from the field samples like Arsenic, Hardness, pH, Iron, Fluoride, TDS, and Sulphate. To assess the ground water quality of the Rupnagar district, total 280 numbers of samples from various sources of tubewells for both pre and post monsoon have collected. Out of which, 80 to 113 samples found Iron with non potable limits ranging 0.3-1.1mg/l and 0.3-1.02mg/l according to BIS standard for both the seasons respectively. Chamkaur Sahib, Rupnagar, Morinda blocks have been found non potable limit of iron in both pre & post-monsoon. 11 to 52 samples in this region have sulphate with permissible limits in both the season ranging 200-400mg/l and 201-400mg/l. But arsenic had acceptable limit in both the season. Various parameters-wise ground water quality map is generated using the range values of drinking water quality to know the distribution of different parameters and diversification in the concentration of different elements. These maps are very much needful for human being to expand awareness among the people to maintain the Cleanness of water at their highest quality and purity levels to achieve a healthy life.

Particulate products are usually characterized by some PSD, shape or porosity parameter(s). For proper characterization, adequate sampling, dispersion, and measurement procedures should be available or developed and skilful personnel should use appropriate, well-calibrated/qualified equipment. The characterization should be executed, in agreement with customers, in a wellorganized laboratory. All related aspects should be laid down in a quality handbook. The laboratory should provide proof for its capability to perform the characterization of stated products and/or reference materials within stated confidence limits. This can be done either by internal validation and audits or by external GLP accreditation.

    Generalized maps showing variations in concentration of chlorides, sulfates, hardness, and dissolved solids in the Floridan aquifer have been prepared as part of a cooperative program with the Southwest Florida Water Management District. This report covers 10 counties and parts of 6 others comprising the District. Data used to develop the report were retrieved from the water-quality files of the U.S. Geological Survey. Considerable vertical and areal variation of chemical constituents was found in ground water of the Floridan aquifer. In general, ground water becomes more mineralized with increasing depth and with increasing distance from recharge areas due to solution of minerals from the aquifer. The model most frequently used for quality management is represented by the ISO 9000: Vision 2000, that is a management model with specific reference to the organization and the customer satisfaction. The model applied to the health structure needs a rapid change in mentality that addresses the operators to define, share and achieve objectives to be brought on by means of an active collaboration, group activity and deep sense of belonging necessary to the attainment of expected objectives. When the model is applied by a scientific society, it is necessary to take into account the different structural and functional organization, the constitution and the operators differing on the point of view of origin, experiences, mentality, and roles. The ISO 9000: Vision 2000 model can be applied also to the cardiovascular echo laboratory which may be compared to a simple organization; for its corrected functioning, SIEC has defined minimal requirements for the accreditation, realization and modalities to carry out and manage quality.     Background Maternal mortality is a major public health problem worldwide especially in low income countries. Most causes of maternal deaths are due to direct obstetric complications. Maternal mortality ratio remains high in Rukungiri district, western Uganda estimated at 475 per 100,000 live births. The objectives were to identify types of community involvement and examine factors influencing the level of community involvement in the management of obstetric emergencies. Methods We conducted a descriptive study during 2nd to 28th February 2009 in rural Rukungiri district, western Uganda. A total of 448 heads of households, randomly selected from 6/11 (54.5%) of sub-counties, 21/42 (50.0%) parishes and 32/212 (15.1%) villages (clusters), were interviewed. Data were analysed using STATA version 10.0. Results Community pre-emergency support interventions available included community awareness creation (sensitization) while interventions undertaken when emergency had occurred included transportation and referring women to health facility. Community support programmes towards health care (obstetric emergencies) included establishment of community savings and credit schemes, and insurance schemes. The factors associated with community involvement in obstetric emergency management were community members being employed (AOR = 1.91, 95% CI: 1.02 - 3.54) and rating the quality of maternal health care as good (AOR = 2.22, 95% CI: 1.19 - 4.14). Conclusions Types of community involvement in obstetric emergency management include practices and support programmes. Community involvement in obstetric emergency management is influenced by employment status and perceived quality of health care services. Policies to promote community networks and resource mobilization strategies for health care should be implemented.     The WMG QAP is an integral part of a management system designed to ensure that WMG activities are planned, performed, documented, and verified in a manner that assures a quality product. A quality product is one that meets all waste acceptance criteria, conforms to all permit and regulatory requirements, and is accepted at the offsite treatment, storage, and disposal facility. In addition to internal processes, this QA Plan identifies WMG processes providing oversight and assurance to line management that waste is managed according to all federal, state, and local requirements for waste generator areas. A variety of quality assurance activities are integral to managing waste. These QA functions have been identified in the relevant procedures and in subsequent sections of this plan. The WMG QAP defines the requirements of the WMG quality assurance program. These requirements are derived from Department of Energy (DOE) Order 414.1C, Quality Assurance, Contractor Requirements Document, the LBNL Operating and Assurance Program Plan (OAP), and other applicable environmental compliance documents. The QAP and all associated WMG policies and procedures are periodically reviewed and revised, as necessary, to implement corrective actions, and to reflect changes that have occurred in regulations, requirements, or practices as a result of feedback on work performed or lessons learned from other organizations. The provisions of this QAP and its implementing documents apply to quality-affecting activities performed by the WMG; WMG personnel, contractors, and vendors; and personnel from other associated LBNL organizations, except where such contractors, vendors, or organizations are governed by their own WMG-approved QA programs.

    An evaluation based on recording the number of patients and evaluating their treatment outcomes provides the information necessary to plan the provision of care, determine the analysis of the situation and revise practice if the results are not satisfactory. The standardised tools proposed for this evaluation are: a district register for new persistent asthma patients, quarterly reports of case finding and an annual report of cohort patient follow-up. The main indicators of quality of care based on register information given by the cohort analysis are the percentage of defaulters and the percentage of patients whose asthma is controlled or well controlled after 1 year of follow-up. The services involved in asthma management should be adapted to the local situation in each country. In particular, the health service structure and national guidelines must be respected, and services involved in asthma management should be implemented in stages. Operational research within the services is essential to ensure that the services provided are appropriate. This type of research involves the health personnel responsible for patient management, provides them with new knowledge and helps them to resolve problems they are confronted with on a regular basis.     In China, national regulations and standards for health care waste management were implemented in 2003. To investigate the current status of health care waste management at different levels of health care facilities (HCF) after the implementation of these regulations, one tertiary hospital, one secondary hospital, and four primary health care centers from Binzhou District were visited and 145 medical staff members and 24 cleaning personnel were interviewed. Generated medical waste totaled 1.22, 0.77, and 1.17 kg/bed/day in tertiary, secondary, and primary HCF, respectively. The amount of medical waste generated in primary health care centers was much higher than that in secondary hospitals, which may be attributed to general waste being mixed with medical waste. This study found that the level of the HCF, responsibility for medical waste management in departments and wards, educational background and training experience can be factors that determine medical staff members' knowledge of health care waste management policy. Regular training programs and sufficient provision of protective measures are urgently needed to improve occupational safety for cleaning personnel. Financing and administrative monitoring by local authorities is needed to improve handling practices and the implementation of off-site centralized disposal in primary health care centers.

    Background An innovative, three-year training programme, the Bachelor of Clinical Medical Practice (BCMP), for mid-level medical healthcare workers was started in 2009 by the Department of Family Medicine, University of Pretoria. Aim To measure the students’ perceptions of the instructional quality of district hospital-based training. Setting Training of students took place at clinical learning centres in rural district hospitals in the Mpumalanga and Gauteng provinces. Methods A survey using the MedEd IQ questionnaire was performed in 2010 and 2011 to measure BCMP second- and third-year students’ perceptions of instructional quality of district hospital-based training. The MedEd IQ questionnaire is composed of four subscales: preceptor activities, learning opportunities, learner involvement and the learning environment. Composite scores of instructional quality were used to present results. Results The preceptor activities, learning opportunities and the learning environment were considered by second- and third-year BCMP students to be of consistently high instructional quality. In the area of learner involvement, instructional quality increased significantly from second to third year. Conclusion Overall, instructional quality of district hospital-based training was high for both second- and third-year BCMP students, and the instructional quality of learner involvement being significantly higher in third year students. The MedEd IQ tool was a useful tool for measuring instructional quality and to inform programme quality improvement. PMID:27543282

    Nationally representative data from the 2012 School Health Policies and Practices Study examined whether state assistance on indoor air quality (IAQ) was associated with district-level policies and practices related to IAQ and integrated pest management (IPM). Districts in states that provided assistance on IAQ were more likely than districts not in such states to (1) have an IAQ management program (p < .001); (2) require schools to conduct periodic inspections of the heating, ventilation, and air conditioning system (p < .05); of the building for cracks, leaks, or past water damage (p < .01); for mold (p < .01); for clutter that prevents effective cleaning and maintenance (p < .05); of the plumbing system (p < .01); and for condensation in and around school facilities (p < .001); (3) have an engine idling reduction program ( < .001); (4) have a policy to purchase low-emitting products (p < .05); and (5) require IPM strategies (p < .05).     Weight-related outcomes were examined among high school students in Minnesota public school districts according to the quality of district wellness policies. Wellness policy strength and comprehensiveness were scored using the Wellness School Assessment Tool (WellSAT) for 325 Minnesota public school districts in 2013. The associations between WellSAT scores and district-level means of high school student responses to a statewide survey of health behaviors were examined in this ecologic study. WellSAT Total Strength and Total Comprehensiveness scores were positively associated with both student mean Body Mass Index (BMI) percentile (Strength: P = 0.018, Comprehensiveness: P = 0.031) and mean percent overweight or obese (Strength: P = 0.008, Comprehensiveness: P = 0.026), but only in districts with > 50% of students eligible for Free or Reduced-Price Lunches (FRPLs), or 'high FRPL districts'. WellSAT Physical Education and Physical Activity subscale scores were also positively associated with the mean days per week students engaged in physical activity for ≥ 60 min in high FRPL districts (Strength: P = 0.008, Comprehensiveness: P = 0.003) and in low FRPL districts (< 35% eligible) for Strength score: (P = 0.027). In medium FRPL districts (35-50% eligible), Nutrition Education and Wellness Promotion Strength and Comprehensiveness subscale scores were positively associated with, respectively, daily servings of vegetables (P = 0.037) and fruit (P = 0.027); and WellSAT Total scores were positively associated with daily vegetable servings (Strength: P = 0.037, Comprehensiveness: P = 0.012). Administrators of economically disadvantaged school districts with a higher percentage of overweight students may be recognizing the need for stronger wellness policies and the specific importance of implementing policies pertaining to physical activity as a means to improve student health.     As government funding grows scarce, competition between the national laboratories is increasing dramatically. In this era of tougher competition, there is no for resistance to change. There must instead be a uniform commitment to improving the overall quality of our products (research and technology) and an increased focus on our customers` needs. There has been an ongoing effort to bring the principles of total quality management (TQM) to all Energy Systems employees to help them better prepare for future changes while responding to the pressures on federal budgets. The need exists for instituting a vigorous program of education and training to an understanding of the techniques needed to improve and initiate a change in organizational culture. The TQM facilitator is responsible for educating the work force on the benefits of self-managed work teams, designing a program of instruction for implementation, and thus getting TQM off the ground at the worker and first-line supervisory levels so that the benefits can flow back up. This program plan presents a conceptual model for TQM in the form of a hot air balloon. In this model, there are numerous factors which can individually and collectively impede the progress of TQM within the division and the Laboratory. When these factors are addressed and corrected, the benefits of TQM become more visible. As this occurs, it is hoped that workers and management alike will grasp the ``total quality`` concept as an acceptable agent for change and continual improvement. TQM can then rise to the occasion and take its rightful place as an integral and valid step in the Laboratory`s formula for survival.

    Performance of local health services managers at district level is crucial to ensure that health services are of good quality and cater to the health needs of the population in the area. In many low- and middle-income countries, health services managers are poorly equipped with public health management capacities needed for planning and managing their local health system. In the south Indian Tumkur district, a consortium of five non-governmental organizations partnered with the state government to organize a capacity-building program for health managers. The program consisted of a mix of periodic contact classes, mentoring and assignments and was spread over 30 months. In this paper, we develop a theoretical framework in the form of a refined program theory to understand how such a capacity-building program could bring about organizational change. A well-formulated program theory enables an understanding of how interventions could bring about improvements and an evaluation of the intervention. In the refined program theory of the intervention, we identified various factors at individual, institutional, and environmental levels that could interact with the hypothesized mechanisms of organizational change, such as staff’s perceived self-efficacy and commitment to their organizations. Based on this program theory, we formulated context–mechanism–outcome configurations that can be used to evaluate the intervention and, more specifically, to understand what worked, for whom and under what conditions. We discuss the application of program theory development in conducting a realist evaluation. Realist evaluation embraces principles of systems thinking by providing a method for understanding how elements of the system interact with one another in producing a given outcome. PMID:25121081

    This study deals with generation, composition, collection, transportation, and disposal, as well as the present cost of the waste management on the basis of 60% collection of the total waste and the cost of proposed improved system of management on the basis of 100% waste collection using the IWM-2 LCI model. A GIS map of Data Ganj Bakhsh Town (DGBT) of Lahore City District showing communal storage facilities is also provided. DGBT has a population of 1,624,169 living in 232,024 dwellings. The total waste generated per year is 500,000 tons, or 0.84/kg/cap/day. Presently 60% of the MSW is collected and disposed in open dumps, while 40% is not collected and lies along roadsides, streets railway lines, depressions, vacant plots, drains, storm drains and open sewers. In DGBT, 129 containers of 5-m{sup 3} capacity, 120 containers of 10-m{sup 3} capacity and 380 skips of 2.5-m{sup 3} capacity are placed for waste collection. The overall collection and disposal cost of the MSW of DGBT is $3,177,900/yr, which is $10.29/ton. Modeling was conducted using the IWM-2 model for improved collection and disposal on the basis of 100% service, compared to the current 60% service. The modelled cost is $8.3/per ton, which is 20% less than the present cost, but the overall cost of 100% collection and disposal increases to $4,155,737/yr.

    Treatment of critical ill patients in the intensive care unit is tantamount to well-designed risk or quality management. Several tools of quality management and quality assurance have been developed in intensive care medicine. In addition to external quality assurance by benchmarking with regard to the intensive care medicine, peer review procedures have been established for external quality assurance in recent years. In the process of peer review of an intensive care unit (ICU), external physicians and nurses visit the ICU, evaluate on-site proceedings, and discuss with the managing team of the ICU possibilities for optimization.     Treatment of critical ill patients in the intensive care unit is tantamount to well-designed risk or quality management. Several tools of quality management and quality assurance have been developed in intensive care medicine. In addition to extern quality assurance by benchmarking with regard to the intensive care medicine, peer review procedures have been established for external quality assurance in recent years. In the process of peer review of an intensive care unit (ICU), external physicians and nurses visit the ICU, evaluate on-site proceedings, and discuss with the managing team of the ICU possibilities for optimization. Furthermore, internal quality management in the ICU is possible based on the 10 quality indicators of the German Interdisciplinary Society for Intensive Care Medicine (DIVI, "Deutschen Interdisziplinären Vereinigung für Intensiv- und Notfallmedizin").   17. Training pharmacy workers in recognition, management, and prevention of STDs: district-randomized controlled trial.

    OBJECTIVES: To determine the effectiveness of an intervention for pharmacy workers in improving their recognition and management of sexually transmitted disease (STD) syndromes. METHODS: We randomly selected 14 districts (total population nearly 4 million) from the 24 districts of low socioeconomic status in Lima, Peru. We randomly assigned paired districts to receive training and support for management and prevention of STDs or a control intervention about management of diarrhoea. The STD intervention included interactive luncheon seminars on recognition and management of four STD syndromes (urethral discharge, vaginal discharge, genital ulcers, and pelvic inflammatory disease) and STD/HIV prevention counselling; monthly pharmacy visits by "prevention salespersons" who distributed materials that included "STD/HIV prevention packets" containing information, condoms, and cards given to patients for referral of their sex partners; and workshops for physicians on managing patients with STD syndromes referred from pharmacies. Standardized simulated patients visited pharmacies in intervention and control districts at one, three, and six months after training to assess outcomes. FINDINGS: Standardized simulated patients reported significantly better recognition and management (appropriate antimicrobial regimens provided for discharge syndromes and referral to specially trained physicians for genital ulcers or pelvic inflammatory disease) by pharmacy workers of all four STD syndromes. They also reported significantly more frequent recommendations for use of condoms and treatment of partners at pharmacies in intervention districts than in control districts (by "intention-to-train" analyses, P<0.05 for 47/48 primary outcome comparisons).     The article deals with two wells in the country of Brno-district (Brčálka well and Well Olšová). The aim of work was monitoring of elementary parameters of water at regular monthly intervals to measure: water temperature, pH values, solubility oxygen and spring yield. According to the client's requirements (Lesy města Brno) laboratory analyzes of selected parameters were done twice a year and their results were compared with Ministry of Health Decree no. 252/2004 Coll.. These parameters: nitrate, chemical oxygen demand (COD), calcium and magnesium and its values are presented in graphs, for ammonium ions and nitrite in the table. Graphical interpretation of spring yields dependence on the monthly total rainfall and dependence of water temperature on ambient temperature was utilized. The most important features of wells include a water source, a landmark in the landscape, aesthetic element or resting and relaxing place. Maintaining wells is important in terms of future generations.

    Background. National VL Elimination Programs in India, Nepal and Bangladesh face challenges as home-based Miltefosine treatment is introduced. Objectives. To study constraints of VL management in endemic districts within context of national elimination programs before and after intervention. Methods. Ninety-two and 41 newly diagnosed VL patients were interviewed for clinical and provider experience in 2009 before and in 2010 after intervention (district training and improved supply of diagnostics and drugs). Providers were assessed for adherence to treatment guidelines. Facilities and doctor-patient consultations were observed to assess quality of care. Results. Miltefosine use increased from 33% to 59% except in Nepal where amphotericin was better available. Incorrect dosage and treatment interruptions were rare. Advice on potential side effects was uncommon but improved significantly in 2010. Physicians did not rule out pregnancy prior to starting Miltefosine. Fever measurement or spleen palpation was infrequently done in Bangladesh but improved after intervention (from 23% to 47%). Physician awareness of renal or liver toxicity as Miltefosine side effects was lower in Bangladesh. Bio-chemical monitoring was uncommon. Patient satisfaction with services remained low for ease of access or time provider spent with patient. Health facilities were better stocked with rK39 kits and Miltefosine in 2010. PMID:22649459

  3. Improving paediatric and neonatal care in rural district hospitals in the highlands of Papua New Guinea: a quality improvement approach

    Background In developing countries such as Papua New Guinea (PNG), district hospitals play a vital role in clinical care, training health-care workers, implementing immunization and other public health programmes and providing necessary data on disease burdens and outcomes. Pneumonia and neonatal conditions are a major cause of child admission and death in hospitals throughout PNG. Oxygen therapy is an essential component of the management of pneumonia and neonatal conditions, but facilities for oxygen and care of the sick newborn are often inadequate, especially in district hospitals. Improving this area may be a vehicle for improving overall quality of care. Method A qualitative study of five rural district hospitals in the highlands provinces of Papua New Guinea was undertaken. A structured survey instrument was used by a paediatrician and a biomedical technician to assess the quality of paediatric care, the case-mix and outcomes, resources for delivery of good-quality care for children with pneumonia and neonatal illnesses, existing oxygen systems and equipment, drugs and consumables, infection-control facilities and the reliability of the electricity supply to each hospital. A floor plan was drawn up for the installation of the oxygen concentrators and a plan for improving care of sick neonates, and a process of addressing other priorities was begun. Results In remote parts of PNG, many district hospitals are run by under-resourced non-government organizations. Most hospitals had general wards in which both adults and children were managed together. Paediatric case-loads ranged between 232 and 840 patients per year with overall case-fatality rates (CFR) of 3–6% and up to 15% among sick neonates. Pneumonia accounts for 28–37% of admissions with a CFR of up to 8%. There were no supervisory visits by paediatricians, and little or no continuing professional development of staff.   7. Strengthening district-based health reporting through the district health management information software system: the Ugandan experience

    Background Untimely, incomplete and inaccurate data are common challenges in planning, monitoring and evaluation of health sector performance, and health service delivery in many sub-Saharan African settings. We document Uganda’s experience in strengthening routine health data reporting through the roll-out of the District Health Management Information Software System version 2 (DHIS2). Methods DHIS2 was adopted at the national level in January 2011. The system was initially piloted in 4 districts, before it was rolled out to all the 112 districts by July 2012. As part of the roll-out process, 35 training workshops targeting 972 users were conducted throughout the country. Those trained included Records Assistants (168, 17.3%), District Health Officers (112, 11.5%), Health Management Information System Focal Persons (HMIS-FPs) (112, 11.5%), District Biostatisticians (107, 11%) and other health workers (473, 48.7%). To assess improvements in health reporting, we compared data on completeness and timeliness of outpatient and inpatient reporting for the period before (2011/12) and after (2012/13) the introduction of DHIS2. We reviewed data on the reporting of selected health service coverage indicators as a proxy for improved health reporting, and documented implementation challenges and lessons learned during the DHIS2 roll-out process. Results Completeness of outpatient reporting increased from 36.3% in 2011/12 to 85.3% in 2012/13 while timeliness of outpatient reporting increased from 22.4% to 77.6%. Similarly, completeness of inpatient reporting increased from 20.6% to 57.9% while timeliness of inpatient reporting increased from 22.5% to 75.6%. There was increased reporting on selected health coverage indicators (e.g. the reporting of one-year old children who were immunized with three doses of pentavelent vaccine increased from 57% in 2011/12 to 87% in 2012/13).     The Picher mining district of northeastern Ottawa County, Oklahoma, was a major site of mining for lead and zinc ores in the first half of the 20th century. The primary source of lead and zinc were sulfide minerals disseminated in the cherty limestones and dolomites of the Boone Formation of Mississippian age, which comprises the Boone aquifer. Ground water in the aquifer and seeping to surface water in the district has been contaminated by sulfate, iron, lead, zinc, and several other metals. The U.S. Geological Survey, in cooperation with the Oklahoma Department of Environmental Quality, investigated hydrology and ground-water quality in the mine workings in the mining district, as part of the process to aid water managers and planners in designing remediation measures that may restore the environmental quality of the district to pre-mining conditions. Most ground-water levels underlying the mining district had similar altitudes, indicating a large degree of hydraulic connection in the mine workings and overlying aquifer materials. Recharge-age dates derived from concentrations of chlorofluorocarbons and other dissolved gases indicated that water in the Boone aquifer may flow slowly from the northeast and southeast portions of the mining district. However, recharge-age dates may have been affected by the types of sites sampled, with more recent recharge-age dates being associated with mine-shafts, which are more prone to atmospheric interactions and surface runoff than the sampled airshafts. Water levels in streams upstream from the confluence of Tar and Lytle Creeks were several feet higher than those in adjacent portions of the Boone aquifer, perhaps due to low-permeability streambed sediments and indicating the streams may be losing water to the aquifer in this area. From just upstream to downstream from the confluence of Tar and Lytle Creeks, surface-water elevations in these streams were less than those in the surrounding Boone aquifer, indicating that

Customer satisfaction is a variable that must be identified for each practice. In my practice, patients perceive quality as communication and time, be it treatment or waiting time. Time is a value and cost that must be managed effectively. Total quality management is a business function; it involves diagnosis, design, implementation, and measurement of the process, the people, and the service.     In the present study, the hydrochemistry and geologic characteristics of the shallow groundwater aquifer along Wadi Al Showat, Khamis Mushiet District, Southwest Saudi Arabia was evaluated and assessed. Along this wadi the fractured/weathered basement rocks house significant quantity of groundwater that usually used by local people for agricultural and domestic purposes. Assessing and evaluation of the quality of the groundwater in such shallow aquifers is very important; especially the groundwater is generally occurred within the fractured basement rocks at shallow depths, thus exposing the groundwater to surface or near-surface contaminants is expected. For this purpose hydrochemical and biological analysis was conducted for 25 water samples collected from the available shallow dug wells along the studied wadi. The study reveals that the groundwater quality changed due to the agriculture and urbanization practices along the wadi. The effect of domestic waste water and septic tanks was obvious. In addition, the field investigation indicates that the basement rocks in the area is dissected by two main sets of fractures that oriented in the west-northwest and east-west directions. In some places, the basement rocks is intruded by coarse-grained, quartz-rich quartzite grained monzogranite, and pegmatite veins that have a coarse-grained weathering product, therefore, they tend to develop and preserve open joint systems between the granitic blocks. These fracturing system are important from the hydrogeological point of view, as they facilitate the storage, water flow movement through them and also facilitate the vertical infiltration of the surface pollutants. These results led to a better understanding of the groundwater characteristics that is important in groundwater management in the study area.

    This paper examines the quality of groundwater of Sabour block, Bhagalpur district of Bihar state, which lies on the southern region of Indo-Gangetic plains in India. Fifty-nine samples from different sources of water in the block have been collected to determine its suitability for drinking and irrigational purposes. From the samples electrical conductivity (EC), pH and concentrations of Calcium (Ca2+), Magnesium (Mg2+), Sodium (Na+), Potassium (K+), carbonate ion (CO{3/2-}), Bicarbonate ion (HCO{3/-}), Chloride ion (Cl-), and Fluoride (F-) were determined. Surface maps of all the groundwater quality parameters have been prepared using radial basis function (RBF) method. RBF model was used to interpolate data points in a group of multi-dimensional space. Root Mean Square Error (RMSE) is employed to scrutinize the best fit of the model to compare the obtained value. The mean value of pH, EC, Ca2+, Mg2+, Na+, K+, HCO3 -, Cl-, and F- are found to be 7.26, 0.69, 38.98, 34.20, 16.92, 1.19, 0.02, and 0.28, respectively. Distribution of calcium concentration is increasing to the eastern part and K+ concentrations raise to the downstream area in the southwestern part. Low pH concentrations (less than 6.71) occur in eastern part of the block. Spatial variations of hardness in Sabour block portraying maximum concentration in the western part and maximum SAR (more than 4.23) were recorded in the southern part. These results are not exceeding for drinking and irrigation uses recommended by World Health Organization. Therefore, the majority of groundwater samples are found to be safe for drinking and irrigation management practices.

    The district health system in Ghana today is characterized by high resource-uncertainty and narrow decision-space. This article builds a theory-driven historical case study to describe the influence of path-dependent administrative, fiscal and political decentralization processes on development of the district health system and district manager decision-space. Methods included a non-exhaustive literature review of democratic governance in Ghana, and key informant interviews with high-level health system officials integral to the development of the district health system. Through our analysis we identified four periods of district health system progression: (1) development of the district health system (1970-85); (2) Strengthening District Health Systems Initiative (1986-93); (3) health sector reform planning and creation of the Ghana Health Service (1994-96) and (4) health sector reform implementation (1997-2007). It was observed that district manager decision-space steadily widened during periods (1) and (2), due to increases in managerial profile, and concerted efforts at managerial capacity strengthening. Periods (3) and (4) saw initial augmentation of district health system financing, further widening managerial decision-space. However, the latter half of period 4 witnessed district manager decision-space contraction. Formalization of Ghana Health Service structures influenced by self-reinforcing tendencies towards centralized decision-making, national and donor shifts in health sector financing, and changes in key policy actors all worked to the detriment of the district health system, reversing early gains from bottom-up development of the district health system. Policy feedback mechanisms have been influenced by historical and contemporary sequencing of local government and health sector decentralization. An initial act of administrative decentralization, followed by incomplete political and fiscal decentralization has ensured that the balance of power has

    A fuzzy fractional chance-constrained programming model (FFCCPM) was developed for dealing with air quality management under uncertainty. FFCCPM integrates a fractional programming model and a double-sided fuzzy chance-constrained programming model. It considers the ratio between total treated pollutant amounts and system cost in the objective function; the constraints with fuzzy variables can be satisfied under some predetermined confidence levels and reliability scenarios. The air quality management system in Fengrun district, Tangshan City, China, was used to demonstrate the applicability of the proposed method. The obtained results indicated that the proposed model was suitable in describing and providing an overview of a studied management system for decision makers, generating various cost-effective air pollution-abatement alternatives. The strategy with a balance between system economy and reliability was recommended for decision makers. The successful application of FFCCPM in Fengrun district provides a good example of real-world regional air quality management.

  20. Improving the quality of weekend handover at Yeovil District Hospital

    “Handover of care is one of the most perilous procedures in medicine” (British Medical Association, Safe Handover, Safe Patients). The system in place for weekend handover at YDH was deemed disorganised, unstructured and frequently missing key pieces of information, leaving the on-call Foundation Year 1 (FY1) doctor with only vague jobs and management plans. Baseline surveys demonstrated that junior doctors felt the system was inadequate, potentially compromised patient safety and increased their stress levels. In order to improve this problem a structured weekend handover proforma was created, comparable with the “Out of hours handover record keeping standards: template” from the Royal College of Physicians. This was made readily accessible on the local intranet. Education sessions were organised for the FY1 and FY2 doctors. The impact of the newly introduced proforma was measured using feedback surveys each week from the FY1 on ward cover for six months. A further change implemented was the introduction of a Friday Ward Round proforma. The aim was to reduce the time required to review notes by the on-call doctor, to minimise avoidable weekend jobs and to improve compliance with the management plans. The results demonstrated 100% compliance with the new proformas. There were notable improvements in the presence of a plan (37.5% to 91.7%, max. 100%), a minimum of two patient identifiers (68.8% to 100%) and relevant background information (62.5% to 100%). Qualitative data showed a much higher level of satisfaction with the new system. Future plans include rolling out electronic handover to improve problems such as illegible handwriting and missing data (enable ‘compulsory’ fields), and also for this system to be implemented Trust-wide.     This paper describes the management approach being taken within the Hazardous Waste Remedial Actions Program (HAZWRAP) Support Contractor Office (SCO) to ensure quality of services in a highly competitive waste management environment. An overview is presented of the contractor support role assigned to Martin Marietta Energy Systems, Inc., by the Department of Energy (DOE) national program for managing hazardous waste. The HAZWRAP SCO mission, organizational structure, and major programs are outlined, with emphasis on waste management planning for the DOE Work for Others (WFO) Program. The HAZWRAP SCO provides waste management technical support, via interagency agreements between DOE and various Department of Defense (DOD) agencies for DOD sponsors planning remedial response actions. The remainder of the paper focuses on how the concept of Total Quality Management is applied to the HAZWRAP Remedial Actions Planning (RAP) Program. The management challenge is to achieve quality on a ''system'' basis where all functional elements of program management synergistically contribute to the total quality of the effort. The quality assurance (QA) program requirements applied to the RAP Program and its subcontractors are discussed.     Managing environmental programs can be effective only when good cost and cost-related management practices are developed and implemented. The Department of Energy`s Office of Environmental Management (EM), recognizing this key role of cost management, initiated several cost and cost-related management activities including the Cost Quality Management (CQM) Program. The CQM Program includes an assessment activity, Cost Quality Management Assessments (CQMAs), and a technical assistance effort to improve program/project cost effectiveness. CQMAs provide a tool for establishing a baseline of cost-management practices and for measuring improvement in those practices. The result of the CQMA program is an organization that has an increasing cost-consciousness, improved cost-management skills and abilities, and a commitment to respond to the public`s concerns for both a safe environment and prudent budget outlays. The CQMA program is part of the foundation of quality management practices in DOE. The CQMA process has contributed to better cost and cost-related management practices by providing measurements and feedback; defining the components of a quality cost-management system; and helping sites develop/improve specific cost-management techniques and methods.

The adoption of a bioreactor landfill for managing the mechanically sorted organic fraction instead of bio-stabilization led to reduction of global warming and fresh water eutrophication by 50% and 10%, respectively. Extraction of recyclables from residual waste led to avoided emissions for particulate matter, acidification and resource depletion impact categories. Marginal energy and the amount of energy recovered from landfill gas marginally affected the LCA results. On the contrary the quality of the recyclables extracted can significantly modify the eco profile of the management schemes.

    Quality management is an ongoing development resulting in consistency products and services and ever increasing customer satisfaction. The ultimum is Total Quality Management. Quality systems and quality management in transfusion medicine have gained considerable attention since the outbreak of the AIDS epidemic. Where product orientation has long been applied through quality control, Good Manufacturing Practice (GMP) principles were introduced, shifting the developments in the direction of process orientation. Globally, and particularly in the more industrialised world people and system orientation has come along with the introduction of the ISO9001 concept. Harmonisation and a degree of uniformity are needed to implement a universally applicable Quality System and related Quality Management. Where the American Association of Blood Banks (AABB) is the professional organisation with the most extensive experience in quality systems in blood transfusion, the European Union and the Council of Europe now are in the process to design a quality system and management applicable to a larger variety of countries, based on a hybrid of current GMP and ISO9001 principles. The International Federation of Red Cross and Red Crescent Societies has developed a more universally to implement Quality Manual, with a pilot project in Honduras. It is recommendable to harmonise the various designs and bring the approaches under one common denominator. PMID:10938970

  20. Development of a district information system for water management planning and strategic decision making

    The overall objective of this work is the development of a District Information System (DIS) which could be used by stakeholders for the purposes of a district day-to-day water management as well as for planning and strategic decisionmaking. The DIS was developed from a GIS-based modeling approach, which integrates a generic crop model and a hydraulic model of the transport/distribution system, using land use maps generated by Landsat TM imagery. The main sub-objectives are: (i) the development of an operational algorithm to retrieve crop evapotranspiration from remote sensing data, (ii) the development of an information system with friendly user interface for the data base, the crop module and the hydraulic module and (iii) the analysis and validation of management scenarios from model simulations predicting the respective behavior. The Lake Karla watershed is used in this study, but the overall methodology could be used as a basis for future analysis elsewhere. Surface Energy Balance Algorithm for Land (SEBAL) was used to derive monthly actual evapotranspiration (ET) values from Landsat TM imagery. Meteorological data from the archives of the Institute for Research and Technology, Thessaly (I.RE.TE.TH) has also been used. The methodology was developed using high quality Landsat TM images during 2007 growing season. Monthly ET values are used as an input to CROPWAT model. Outputs of CROPWAT model are then used as input for WEAP model. The developed scenario is based on the actual situation of the surface irrigation network of the Local Administration of Land Reclamation (LALR) of Pinios for the year of 2007. The DIS is calibrated with observed data of this year and the district parameterization is conducted based on the actual operation of the network. The operation of the surface irrigation network of Pinios LALR is simulated using Technologismiki Works, while the operation of closed pipe irrigation network of Lake Karla LALR is simulated using Watercad.     Quality management has become one of the most important and most debated topics within the service sector. This is especially true for health care, as the controversy rages on how the existing American system should be restructured. Health care reform aimed at reducing costs and ensuring access to all Americans cannot be allowed to jeopardize the quality of care. As such, total quality management (TQM) has become a vital ingredient to strategic planning within the health care domain. At the heart of any such quality improvement effort is the issue of measurement. TQM cannot be effectively utilized as a competitive weapon unless quality can be accurately defined, measured, evaluated, and monitored over time. Through such analysis a hospital can elect how to expend its limited resources toward those quality improvement projects which will impact customer perceptions of service quality the most. Thus, the purpose of this report is to establish a framework by which to approach the issue of quality measurement, delineate the various components of quality that exist in health care, and explore how these elements affect one another. We propose that the issue of quality measurement in health care be approached as an integration of service quality attributes common to other service organizations and technical quality attributes unique to health care. We hope that this research will serve as a first step toward the synthesis of the various quality attributes inherent in the health care domain and encourage other researchers to address the interactions of the various quality attributes.   6. Deterioration of coastal groundwater quality in Island and mainland regions of Ramanathapuram District, Southern India.

    A study was carried out in the Island and mainland regions of Ramanathapuram District to characterize the physico-chemical characteristics of 87 groundwater samples in Island and 112 groundwater samples in mainland which include pH, EC, TDS, salinity, total alkalinity, calcium hardness, magnesium hardness, total hardness, chloride and fluoride. Heavy inorganic load in majority of the groundwater samples has been estimated due to the salinity, TDS, TH and chloride beyond the threshold level which substantiates the percolation of sea water into the freshwater confined zones. Although the groundwater sources are available in plenty, the scarcity of potable water is most prevalent in this coastal area. The Water Quality Index (WQI) and Langeleir Saturation Index (LSI) have also been calculated to know the potable and corrosive/incrusting nature of the water samples. The statistical tools such as principal component analysis, box plots and correlation matrix have also been used to explain the influence of different physico-chemical parameters with respect to one another among the groundwater samples. The percentage of groundwater samples in mainland was more than that in Island with respect to the acceptable limit of WHO drinking standard, especially in TDS, CH, TH and chloride but the converse is observed in the case of fluoride. About 8% of the mainland aquifers and 42% of Island aquifers were identified to have fluoride greater than 1.5 mg/l. The signature of salt-water intrusion is observed from the ratio of Cl/CO(3)(2-) + HCO(3) and TA/TH. A proper management plan to cater potable water to the immediate needs of the people is to be envisaged. PMID:22527453

  7. Drinking water quality management: a holistic approach.

    A growing list of water contaminants has led to some water suppliers relying primarily on compliance monitoring as a mechanism for managing drinking water quality. While such monitoring is a necessary part of drinking water quality management, experiences with waterborne disease threats and outbreaks have shown that compliance monitoring for numerical limits is not, in itself, sufficient to guarantee the safety and quality of drinking water supplies. To address these issues, the Australian National Health and Medical Research Council (NHMRC) has developed a Framework for Management of Drinking Water Quality (the Framework) for incorporation in the Australian Drinking Water Guidelines, the primary reference on drinking water quality in Australia. The Framework was developed specifically for drinking water supplies and provides a comprehensive and preventive risk management approach from catchment to consumer. It includes holistic guidance on a range of issues considered good practice for system management. The Framework addresses four key areas: Commitment to Drinking Water Quality Management, System Analysis and System Management, Supporting Requirements, and Review. The Framework represents a significantly enhanced approach to the management and regulation of drinking water quality and offers a flexible and proactive means of optimising drinking water quality and protecting public health. Rather than the primary reliance on compliance monitoring, the Framework emphasises prevention, the importance of risk assessment, maintaining the integrity of water supply systems and application of multiple barriers to assure protection of public health. Development of the Framework was undertaken in collaboration with the water industry, regulators and other stakeholder, and will promote a common and unified approach to drinking water quality management throughout Australia.   8. Characterization of Ground-Water Quality, Upper Republican Natural Resources District, Nebraska, 1998-2001

    Nearly all rural inhabitants and livestock in the Upper Republican Natural Resources District (URNRD) in southwestern Nebraska use ground water that can be affected by elevated nitrate concentrations. The development of ground-water irrigation in this area has increased the vulnerability of ground water to the introduction of fertilizers and other agricultural chemicals. In 1998, the U.S. Geological Survey, in cooperation with the Upper Republican Natural Resources District, began a study to characterize the quality of ground water in the Upper Republican Natural Resources District area with respect to physical properties and concentrations of major ions, coliform bacteria, nitrate, and pesticides, and to assess the presence of nitrogen concentrations in the unsaturated zone. At selected well sites, the ground-water characterization also included tritium and nitrogen-isotope analyses to provide information about the approximate age of the ground water and potential sources of nitrogen detected in ground-water samples, respectively. In 1998, ground-water samples were collected from 101 randomly selected domestic-well sites. Of the 101 samples collected, 26 tested positive for total coliform bacteria, exceeding the U.S. Environmental Protection Agency's Maximum Contaminant Level (MCL) of zero colonies. In 1999, ground-water samples were collected from 31 of the 101 well sites, and 16 tested positive for coliform bacteria. Nitrates were detected in ground water from all domestic-well samples and from all but four of the irrigation-well samples collected from 1998 to 2001. Eight percent of the domestic-well samples and 3 percent of the irrigation-well samples had nitrate concentrations exceeding the U.S. Environmental Protection Agency's MCL for drinking water of 10 milligrams per liter. For this purpose, twenty-five surface water samples were collected from river, ponds and canals and analysed for pH, electrical conductivity, total dissolved solids (TDS), turbidity, hardness, major cations (Ca2+, Mg2+, Na+ and K+), major anions (HCO3 -, F-, Cl-, NO3 -, SO4 2-) and dissolved silica concentration. The analytical results show mildly acidic to alkaline nature of surface water resources of Pratapgarh district. HCO3 - and Cl- are the dominant anions, while cation chemistry is dominated by Na+ and Ca2+. The statistical analysis and data plotted on the Piper diagram reveals that the surface water chemistry is mainly controlled by rock weathering with secondary contributions from agriculture and anthropogenic sources. Ca2+-Mg2+-HCO3 -, Ca2+-Mg2+-Cl- and Na+-HCO3 --Cl- are the dominant hydrogeochemical facies in the surface water of the area. For quality assessment, values of analysed parameters were compared with Indian and WHO water quality standards, which shows that the concentrations of TDS, F-, NO3 -, Na+, Mg2+ and total hardness are exceeding the desirable limits in some water samples. Water Quality Index (WQI) is one of the most effective tools to communicate information on the quality of any water body. The computed WQI values of Pratapgarh district surface water range from 28 to 198 with an average value of 82, and more than half of the study area is under excellent to good category.

  14. Integrated groundwater quality management in urban areas

    Traditionally, groundwater assessments and remediations are approached at the scale of individual groundwater plumes. In urban areas, however, this management of individual groundwater plumes is often problematic for technical, practical or financial reasons, since the groundwater quality is often affected by a combination of sources, including (former) industrial activities, spills and leachate from uncontrolled landfills and building materials. As a result, often a whole series of intermingling contamination plumes is found in large volumes of groundwater. In several countries in the world, this led to stagnation of groundwater remediation in urban areas. Therefore, in the Netherlands there is a tendency managing groundwater in urban areas from an integrated perspective and on a larger scale. This so-called integrated groundwater quality management is often more efficient and hence, cheaper, since the organisation of the management of a cluster of groundwater plumes is much easier than it would be if all individual groundwater plumes were managed at different points in time. Integrated groundwater quality management should follow a tailor-made approach. However, to facilitate practical guidance was developed. This guidance relates to the delineation of the domain, the management of sources for groundwater contamination, procedures for monitoring, and (risk-based) assessment of the groundwater quality. Function-specific risk-based groundwater quality criteria were derived to support the assessment of the groundwater quality.

    Starting from an existing waste management district without thermal treatment facilities, two different management scenarios for residual waste were compared by life cycle assessment (LCA). The adoption of a bioreactor landfill for managing the mechanically sorted organic fraction instead of bio-stabilization led to reduction of global warming and fresh water eutrophication by 50% and 10%, respectively. Extraction of recyclables from residual waste led to avoided emissions for particulate matter, acidification and resource depletion impact categories. Marginal energy and the amount of energy recovered from landfill gas marginally affected the LCA results. On the contrary the quality of the recyclables extracted can significantly modify the eco profile of the management schemes. PMID:26184897

Over the past several years, a number of developments have taken place to respond to these concerns. This quality movement in managed care has not been without some issues and considerations, however. This article first describes the National Committee for Quality Assurance (NCQA) and the prominent role it has played in this movement. Next, quality improvement study design is addressed in the context of assuring quality, controlling costs, and achieving NCQA accreditation. The effect that capitation, as a payment strategy for providers, has on data quality is then described. Fourth, the value of partnering is explored.     The purpose of this article is to demonstrate how the use of a participatory health care management and problem-related research methodology may help strengthen health service management and the capacities of local health care personnel. The text refers to five studies carried out in three Bolivian health districts from 1993-1995, in an Operational Research Project, conducted by an Italian N.G.O. (Cooperazione Internazionale) in agreement with the local Ministry of Health and P.A.H.O./W.H.O. The object of these studies was to assess the main problems in health care delivery and to define and implement appropriate solutions. The studies utilized a methodology based on the principles of operational research and continuous quality improvement. During the process some positive aspects and difficulties were met. The positive aspects were: the applied character of the research focused on the solution of a problem; the study of a problem related to health service management; the use of modern and simple techniques adapted to local knowledge; on-the-job training of health care personnel during the research process. Lack of a generic 'culture of research' and poor health personnel training were the main difficulties encountered. National health authorities should take these points into account to define or readjust policies on health service research, health workers' academic education and in-service training. Insisting on developing human resources and allowing them to achieve and expand their potential is the key factor for getting developing countries out of their current crisis and toward reaching a truly human and sustainable development.     Lack of safe drinking water is a major problem in developing countries. Within Africa most people rely mainly on local groundwater sources for their water needs. These can be deep boreholes or shallow wells, the latter are normally found in poorer communities as they are the least expensive to construct. Over time water from these sources can be contaminated leading to fatal consequences. Previous monitoring of the quality of water from boreholes and shallow wells have been irregular, with the focus being mainly on boreholes. Information on seasonal water quality changes in shallow wells used by rural communities in Malawi has generally been lacking. A study was conducted from 2006 to 2007 to determine the quality of water from shallow wells in three districts in southern Malawi namely, Balaka, Chikwawa and Zomba districts. Water samples from 21 covered and five open shallow wells were analysed for chemical, microbiological and physical parameters using a portable water testing kit. Sampling was carried out at four different times of the year i.e. in August and October 2006 (dry season) and February and April 2007 (wet season). Microbiological data indicated that around 80% of the samples, obtained from the covered wells, failed to meet safe drinking water limits, set by World Health Organisation guidelines and Malawi Bureau of Standards, of zero total and faecal colony forming units (cfu)/100 ml. Values in excess of 1000 cfu/100 ml were noted in 10% of the samples, indicating gross contamination and the probability of pathogens being present. Contamination levels were higher during the wet season than the dry season in all three districts. Arsenic, ammonia, nitrate, nitrite and sulphate were all within the acceptable limits. Elevated levels of hardness, turbidity were noted in certain wells.

    Groundwater quality is important as it is the main factor determining its suitability for drinking, domestic, agricultural and industrial purposes. The suitability of groundwater for drinking and irrigation has been assessed in north and eastern part of Krishnagiri district, South-western part of Vellore district and contiguous with Andhra Pradesh states, India. A total of 31 groundwater samples were collected in the study area. The groundwater quality assessment has been carried out by evaluating the physicochemical parameters such as pH, EC, TDS, HCO3^{ - } , Cl-, SO4^{2 - } , Ca2+, Mg2+, Na+ and K+. The dominant cations are in the order of Na+ > K+ > Ca2+ > Mg2+ while the dominant anions have the trends of Cl- > HCO3^{ - } > SO4^{2 - } > CO3. The quality of the water is evaluated using Wilcox diagram and the results reveals that most of the samples are found to be suitable for irrigation. Based on these parameters, groundwater has been assessed in favor of its suitability for drinking and irrigation purpose.

  20. Utilizing multiobjective analysis to determine an air quality monitoring network in an industrial district

    An industrial district with polluting factories operating inside poses a potential threat to the air quality in the surrounding areas. Therefore, establishing a proper air quality monitoring network (AQMN) is essential for assessing the effectiveness of imposed pollution controls, strategies, and facilities in reducing pollutants. The geographic layout of such an AQMN should assure the quality of the monitored data. Monitoring stations located at inappropriate sites will likely affect data validity. In this study, a multiobjective approach was explored for configuring an AQMN for an industrial district. A dispersion model was employed to simulate hourly distribution of pollutant concentrations in the study area. Models optimizing pollution detection, dosage, coverage, and population protection were established. Alternative AQMNs with varied station numbers and spatial distributions were obtained using the models. The resulting AQMNs were compared and evaluated for effectiveness in monitoring the temporal and spatial variation of pollutants. Discussion of the differences among the AQMNs is provided. This multiobjective analysis is expected to facilitate a decision-making process for determining an appropriate AQMN.

There is an increasing awareness of and interest in the quality of care and patient safety in medical imaging and interventional radiology. Among the professional organisations, a range of quality systems are available to address individual, facility and system needs. To manage the limited resources successfully, radiologists and professional organisations must be leaders and champion for the cause of quality care and patient safety. Close collaboration with other stakeholders towards the development and management of proactive, long-term, system-based strategies and infrastructures will underpin a sustainable future in quality radiology.     Background Maternal and newborn mortality remain unacceptably high in sub-Saharan Africa. Tanzania and Uganda are committed to reduce maternal and newborn mortality, but progress has been limited and many essential interventions are unavailable in primary and referral facilities. Quality management has the potential to overcome low implementation levels by assisting teams of health workers and others finding local solutions to problems in delivering quality care and the underutilization of health services by the community. Existing evidence of the effect of quality management on health worker performance in these contexts has important limitations, and the feasibility of expanding quality management to the community level is unknown. We aim to assess quality management at the district, facility, and community levels, supported by information from high-quality, continuous surveys, and report effects of the quality management intervention on the utilization and quality of services in Tanzania and Uganda. Methods In Uganda and Tanzania, the Expanded Quality Management Using Information Power (EQUIP) intervention is implemented in one intervention district and evaluated using a plausibility design with one non-randomly selected comparison district. The quality management approach is based on the collaborative model for improvement, in which groups of quality improvement teams test new implementation strategies (change ideas) and periodically meet to share results and identify the best strategies. The teams use locally-generated community and health facility data to monitor improvements. In addition, data from continuous health facility and household surveys are used to guide prioritization and decision making by quality improvement teams as well as for evaluation of the intervention.     In many watersheds of the Mediterranean Countries, water resources are presently fully or overcommitted. Irrigators are the largest consumers of fresh water in Mediterranean Countries using up to 80% of all allocated water in some regions. Administrative efforts should be directed towards an integrated policy of water allocation which accounts for the characteristics and specificity of each farm, requiring the availability of data bases and management tools (decision support systems) specifically designed to fulfil the objectives of maximizing water use efficiency. The overall objective of this program was the development of a District Information System (DIS) which could be used by stakeholders at purposes of irrigation district day-to-day management as well as for planning and strategic decision-making. The DIS was developed from a GIS-based modelling approach which integrates a generic crop model, a hydraulic module for the water transfer/distribution system and uses remote sensing information. The main sub-objectives were: (i) the development of an operational algorithm to retrieve crop evapotranspiration from remote sensing data, (ii) the development of an information system with friendly user interface for the data base, the crop module and the hydraulic module and (iii) the analysis and validation of management scenarios from model simulations predicting the respective behaviour. Surface Energy Balance Algorithm for Land (SEBAL) was used to derive monthly actual evapotranspiration (ET) values from Landsat TM imagery. Meteorological data from the archive of the Institute for Research and Technology, Thessaly (I.RE.TE.TH) have also been used.   9. Urban morphology and air quality in dense residential environments in Hong Kong. Part I: District-level analysis

    The link between urban morphology and air quality in various dense residential environments of Hong Kong are investigated through field measurements and statistical analysis. With regard to the myriads of parameters available in the literature, this study aims to identify the most important urban morphological parameters in the street-level and district levels that affect air quality dispersion in Hong Kong. The study postulates that there is an association between urban morphology and urban air quality and a statistical model is developed to explain the relationship amongst urban air quality, micro-meteorology and urban morphological dimension. The study considered 20 different urban residential areas in five major districts of Hong Kong and real-time street-level air pollutant and microclimatic data are collected. 21 morphological variables are identified and calculated based on the geometry of the urban fabric. This study is the first part of a series which focuses to identify whether district demarcation is correlated with air quality. According to the findings of statistical analysis, only 6 out of 21 morphological variables (complete aspect ratio, occlusivity, roughness height, zero-plane displacement height, total building volume/number of buildings, and standard deviation of height) report distinguishable variability at the district level. Their R2 (correlation coefficient) values vary from 0.2937 (plan area index) to 0.5457 (occlusivity) which is considered moderate to high. All these parameters describe the overall feature of the site rather than the specific geometric features of the street. It translates to the fact that in spite of the large in-district variations in morphological variables amongst the five districts, air pollution variations are insignificant for most of the variables and it means that air quality in Hong Kong are distinguishable not by district-level variable but rather in-site fabrics.
