    Lawrence Berkeley Laboratory`s Environment Department addresses its responsibilities through activities in a variety of areas. The need for a comprehensive management control system for these activities has been identified by the Department of Energy (DOE). The WM QA (Waste Management Quality Assurance) Plan is an integral part of a management system that provides controls necessary to ensure that the department`s activities are planned, performed, documented, and verified. This WM QA Plan defines the requirements of the WM QA program. These requirements are derived from DOE Order 5700.6C, Quality Assurance, the LBL Operating and Assurance Program Plan (OAP, LBL PUB-3111), and other environmental compliance documents applicable to WM activities. The requirements presented herein, as well as the procedures and methodologies that direct the implementation of these requirements, will undergo review and revisions as necessary. The provisions of this QA Plan and its implementing documents apply to quality-affecting activities performed by and for WM. It is also applicable to WM contractors, vendors, and other LBL organizations associated with WM activities, except where such contractors, vendors, or organizations are governed by their own WM-approved QA programs. References used in the preparation of this document are (1) ASME NQA-1-1989, (2) ANSI/ASQC E4 (Draft), (3) Waste Management Quality Assurance Implementing Management Plan (LBL PUB-5352, Rev. 1), (4) LBL Operating and Assurance Program Plan (OAP), LBL PUB-3111, 2/3/93. A list of terms and definitions used throughout this document is included as Appendix A.

    The WMG QAP is an integral part of a management system designed to ensure that WMG activities are planned, performed, documented, and verified in a manner that assures a quality product. A quality product is one that meets all waste acceptance criteria, conforms to all permit and regulatory requirements, and is accepted at the offsite treatment, storage, and disposal facility. In addition to internal processes, this QA Plan identifies WMG processes providing oversight and assurance to line management that waste is managed according to all federal, state, and local requirements for waste generator areas. A variety of quality assurance activities are integral to managing waste. These QA functions have been identified in the relevant procedures and in subsequent sections of this plan. The WMG QAP defines the requirements of the WMG quality assurance program. These requirements are derived from Department of Energy (DOE) Order 414.1C, Quality Assurance, Contractor Requirements Document, the LBNL Operating and Assurance Program Plan (OAP), and other applicable environmental compliance documents. The QAP and all associated WMG policies and procedures are periodically reviewed and revised, as necessary, to implement corrective actions, and to reflect changes that have occurred in regulations, requirements, or practices as a result of feedback on work performed or lessons learned from other organizations. The provisions of this QAP and its implementing documents apply to quality-affecting activities performed by the WMG; WMG personnel, contractors, and vendors; and personnel from other associated LBNL organizations, except where such contractors, vendors, or organizations are governed by their own WMG-approved QA programs.

    As government funding grows scarce, competition between the national laboratories is increasing dramatically. In this era of tougher competition, there is no for resistance to change. There must instead be a uniform commitment to improving the overall quality of our products (research and technology) and an increased focus on our customers` needs. There has been an ongoing effort to bring the principles of total quality management (TQM) to all Energy Systems employees to help them better prepare for future changes while responding to the pressures on federal budgets. The need exists for instituting a vigorous program of education and training to an understanding of the techniques needed to improve and initiate a change in organizational culture. The TQM facilitator is responsible for educating the work force on the benefits of self-managed work teams, designing a program of instruction for implementation, and thus getting TQM off the ground at the worker and first-line supervisory levels so that the benefits can flow back up. This program plan presents a conceptual model for TQM in the form of a hot air balloon. In this model, there are numerous factors which can individually and collectively impede the progress of TQM within the division and the Laboratory. When these factors are addressed and corrected, the benefits of TQM become more visible. As this occurs, it is hoped that workers and management alike will grasp the ``total quality`` concept as an acceptable agent for change and continual improvement. TQM can then rise to the occasion and take its rightful place as an integral and valid step in the Laboratory`s formula for survival.

    This paper describes the management approach being taken within the Hazardous Waste Remedial Actions Program (HAZWRAP) Support Contractor Office (SCO) to ensure quality of services in a highly competitive waste management environment. An overview is presented of the contractor support role assigned to Martin Marietta Energy Systems, Inc., by the Department of Energy (DOE) national program for managing hazardous waste. The HAZWRAP SCO mission, organizational structure, and major programs are outlined, with emphasis on waste management planning for the DOE Work for Others (WFO) Program. The HAZWRAP SCO provides waste management technical support, via interagency agreements between DOE and various Department of Defense (DOD) agencies for DOD sponsors planning remedial response actions. The remainder of the paper focuses on how the concept of Total Quality Management is applied to the HAZWRAP Remedial Actions Planning (RAP) Program. The management challenge is to achieve quality on a ''system'' basis where all functional elements of program management synergistically contribute to the total quality of the effort. The quality assurance (QA) program requirements applied to the RAP Program and its subcontractors are discussed. The application of management principles in the areas of program management, procurement, and QA to achieve total quality is presented. 3 refs.

    On July 13, 1998, the U.S. Department of Energy, Richland Operations Office (DOE-RL) Manager transmitted a letter to Fluor Daniel Hanford, Inc. (FDH) describing several DOE-RL identified failed opportunities for FDH to improve the Quality Assurance (QA) Program and its implementation. In addition, DOE-RL identified specific Quality Program performance deficiencies. FDH was requested to establish a periodic reporting mechanism for the corrective action program. In a July 17, 1998 response to DOE-RL, FDH agreed with the DOE concerns and committed to perform a comprehensive review of the Project Hanford Management Contract (PHMC) QA Program during July and August, 1998. As a result, the Project Hanford Management Contract Quality Improvement Plan (QIP) (FDH-3508) was issued on October 21, 1998. The plan identified corrective actions based upon the results of an in-depth Quality Program Assessment. Immediately following the scheduled October 22, 1998, DOE Office of Enforcement and Investigation (EH-10) Enforcement Conference, FDH initiated efforts to effectively implement the QIP corrective actions. A Quality Improvement Project (QI Project) leadership team was assembled to prepare a Project Management Plan for this project. The management plan was specifically designed to engage a core team and the support of representatives from FDH and the major subcontractors (MSCs) to implement the QIP initiatives; identify, correct, and provide feedback as to the root cause for deficiency; and close out the corrective actions. The QI Project will manage and communicate progress of the process.

    Incorporation of aesthetic considerations in the process of landscape planning and development has frequently met with poor results due to its lack of theoretical basis, public involvement, and failure to deal with spatial implications. This problem has been especially evident when dealing with large areas, for example, the Adirondacks, Scenic Highways, and National Forests and Parks. This study made use of public participation to evaluate scenic quality in a portion of the Niagara Escarpment in Southern Ontario, Canada. The results of this study were analyzed using the visual management model proposed by Brown and Itami (1982) as a means of assessing and evaluating scenic quality. The map analysis package formulated by Tomlin (1980) was then applied to this assessment for the purpose of spatial mapping of visual impact. The results of this study illustrate that it is possible to assess visual quality for landscape/management, preservation, and protection using a theoretical basis, public participation, and a systematic spatial mapping process.

    S.M. Stoller Corporation is the contractor for the Technical Assistance Contract (TAC) for the U.S. Department of Energy (DOE) Office of Legacy Management (LM) operations. Stoller employs a management system that applies to all programs, projects, and business management systems funded through DOE-LM task orders. The management system incorporates the philosophy, policies, and requirements of health and safety, environmental compliance, and quality assurance (QA) in all aspects of project planning and implementation. Health and safety requirements are documented in the Health and Safety Manual (STO 2), the Radiological Control Manual (STO 3), the Integrated Safety Management System Description (STO 10), and the Drilling Health and Safety Requirements (STO 14). Environmental compliance policy and requirements are documented in the Environmental Management Program Implementation Manual (STO 11). The QA Program is documented in the Quality Assurance Manual (STO 1). The QA Manual (STO 1) implements the specific requirements and philosophy of DOE Order 414.1C, Quality Assurance. This manual also includes the requirements of other standards that are regularly imposed by customers, regulators, or other DOE orders. Title 10 Code of Federal Regulations Part 830, “Quality Assurance Requirements,” ANSI/ASQC E4-2004, “Quality Systems for Environmental Data and Technology Programs – Requirements with Guidance for Use,” and ISO 14001-2004, “Environmental Management Systems,” have been included. These standards are similar in content. The intent of the QA Manual (STO 1) is to provide a QA management system that incorporates the requirements and philosophy of DOE and other customers within the QA Manual. The QSR requires that ER Program participants develop Quality Management Plans (QMPs) that describe how the QSR requirements will be implemented for their assigned scopes of work. This standard review plan (SRP) describes the ER program participant responsibilities for submittal of QMPs to the RL Environmental Restoration Division for review and the RL methodology for performing the reviews of participant QMPS.     The Environmental Protection Agency's (EPA) Office of Emergency and Remedial Response (OERR) requires environmental data of known quality to support Superfund hazardous waste site projects. The Quality Assurance Technical Support (QATS) Program is operated by Shaw Environmental and Infrastructure, Inc. to provide EPA's Analytical Operations Center (AOC) with performance evaluation samples, reference materials, on-site laboratory auditing capabilities, data audits (including electronic media data audits), methods development, and other support services. The new QATS contract awarded in November 2000 required that the QATS Program become ISO 9000 certified. In a first for an EPA contractor, the QATS staff and management successfully transformed EPA's QA/R-2 type Quality Management Plan into a Quality Management System (QMS) that complies with the requirements of the internationally recognized ISO 9002 standard and achieved certification in the United States, Canada, and throughout Europe. The presentation describes how quality system elements of ISO 9002 were implemented on an already existing quality system. The psychological and organizational challenges of the culture change in QATS' day-to-day operations will be discussed for the benefit of other ISO 9000 aspirants. PMID:12553083

    Objective To examine whether low-income parents of children enrolled in the New York State Children's Health Insurance Program (SCHIP) choose managed care plans with better quality of care. Data Sources 2001 New York SCHIP evaluation data; 2001 New York State Managed Care Plan Performance Report; 2000 New York State Managed Care Enrollment Report. Study Design Each market was defined as a county. A final sample of 2,325 new enrollees was analyzed after excluding those in markets with only one SCHIP plan. Plan quality was measured using seven Consumer Assessment of Health Plans Survey (CAHPS) and three Health Plan Employer Data and Information Set (HEDIS) scores. A conditional logit model was applied with plan and individual/family characteristics as covariates. Principle Findings There were 30 plans in the 45 defined markets. The choice probability increased 2.5 percentage points for each unit increase in the average CAHPS score, and the association was significantly larger in children with special health care needs. However, HEDIS did not show any statistically significant association with plan choice. Conclusions Low-income parents do choose managed care plans with higher CAHPS scores for their newly enrolled children, suggesting that overall quality could improve over time because of the dynamics of enrollment. Experiences using LANDSAT to identify nonpoint sources of water pollution as well as land/use/land cover features in South Dakota, Kentucky, Georgia, New Jersey, and Texas are described. Present activities suggest that this type of remote sensing is an efficient, effective tool for areawide water quality planning.     In Taiwan, the authorities have spent years working on remedying polluted rivers. Generally, the remediation planning works are divided into two phases. During the first phase, the allowed pollution discharge quantity and abatement quantity of each drainage zone, including the assimilative capacity, are generated based on the total river basin. In the second phase, the abatement action plans for each pollution source in each drainage zone are respectively devised by the related organizations based on the strategies generated during the first phase. However, the effectiveness of linking the two phases is usually poor. Highly integrated performances are not always achieved because the separate two-phase method does not take system and management thinking into consideration in the planning stage. This study pioneers the use of the Managing for Results (MFR) method in planning strategies and action plans for river water quality management. A sustainable management framework is proposed based on the concept and method of MFR, Management Thinking, and System Analysis. The framework, consisting of planning, implementation, and controlling stages, systematically considers the relationships and interactions among four factors: environment, society, economy, and institution, based on the principles of sustainable development. Based on the framework, the Modified Bounded Implicit Enumeration algorithm, which is used as a solving method, is combined with Visual Basic software and MS Excel to develop a computer system for strategy planning. The Shetzu River, located in northern Taiwan, is applied as a case study. According to the theoretical, practical, and regulatory considerations, the result-oriented objectives are defined to first improve the pollution length of the Shetzu River in specific remediation periods to finally meet regulated water quality standards.     A parking and routing information system (PARIS) is being designed and deployed at a test site on the Mountain Home Veterans Administration campus in Johnson City, Tennessee using three sensor technologies. The purpose of the PARIS project is to demonstrate innovative integration of vehicle sensing technologies with parking management strategies to improve mobility and relieve congestion associated with a growing medical technology complex. Oak Ridge National Laboratory`s (ORNL) role during phase 1 of the project is to act as the independent evaluator. This technical memorandum presents two plans, Data Management Plan and Quality Control/Quality Assurance (QC/QA) Plan, which were developed to support ORNL`s responsibilities and functions. The purposes of the Data Management Plan are to coordinate the data needs which will be addressed in four individual evaluation test plans and to deal with the data issues facing the phase 1 evaluation as a whole. The objectives of the QC/QA Plan are to ensure that the evaluation is conducted properly, to guarantee that the data which will be collected are valid and properly analyzed, and to make sure that the test results are accurately recorded and reported. The evaluation will be performed through the execution of four integrated studies (system performance, user acceptance, institutional and business issues, and transportation systems). The phase 1 evaluation data will be collected from 24 sources. Three databases will be established based upon these data (project team database, distributed database processor, and evaluator database).

Both WHC-SP-1131,Hanford Quality Assurance Program and Implementation Plan, and WHC-CM-4-2, Quality Assurance Manual, were adopted; however, it was recognized that revisions were required to address the functions and responsibilities of the Project Hanford Management Contract (PHMC). This Quality Assurance Program Implementation Plan for Nuclear Facilities (HNF-SP-1228) supersedes the implementation portion of WHC-SP-1 13 1, Rev. 1. The revised Quality Assurance (QA) Program is documented in the Project Hanford Quality Assurance Program Description (QAPD), HNF-MP-599. That document replaces the QA Program in WHC-SP-1131, Rev. 1. The scope of this document is limited to documenting the nuclear facilities managed by FDH and its Major Subcontractors (MSCS) and the status of the implementation of 10 CFR 830.120, Quality Assurance Requirements, at those facilities. Since the QA Program for the nuclear facilities is now documented in the QAPD, future updates of the information provided in this plan will be by letter.     A growing number of US federal funding agencies require data management plans (DMP) as part of new research grant proposals. To help researchers with this requirement, several organizations (the California Digital Library, University of Illinois, University of Virginia, Smithsonian Institution, the DataONE consortium and the (UK) Digital Curation Centre) came together to develop the DMPTool in 2011. The goal of the DMPTool is to provide researchers with guidance, links to resources and help with writing data management plans. Thanks to a grant from the Alfred P. Sloan Foundation, these organizations have been able to develop DMPTool2, adding new features and functionality, while aiming to grow the number of users and funding requirements and build a community around DMP best practices. Researchers create plans in the tool by selecting their desired funding agency. The tool provides specific requirements from the selected agency as well as detailed help with each area of the plan. Users have access to complete DMP life cycle management, tracking changes through creation, editing, submission, evaluation, and publication. They may also perform enhanced keyword searches and view publicly available plans. With role-based user authorization, users may hold various roles within the interface: creators, collaborators, institutional administrators, and tool administrators. Furthermore, partner institutions can add significant value to the process with several special tool features. Along with institutional branding in the interface, they can provide links to resources they provide to users, such as to preservation repositories, consultation services, or news and event items. In addition, partner institutions can provide help with specific plan questions, and even suggest responses.     Planning for the introduction, implementation, and conduct of quality assurance activities has been the key issue from the outset of the project. Despite the various approaches to planning, no single Quality Assurance (QA) planning can be universally accepted by developing countries due to variations in the socioeconomic, cultural and political makeup of individual countries. This paper summarizes the lessons learned from the Quality Assurance Project in planning a QA program: 1) the need to understand organizational strengths and weaknesses to develop appropriate strategies for QA skills training and organizational change; 2) the need to build on existing systems or activities that support the objectives of the organization and provide an adequate foundation for the QA program; 3) the need to assign responsibility for quality assurance through the creation of QA councils and committees and the assignment of coordinators and other individuals; 4) the need to secure top-level management support to legitimize any changes; 5) the need to determine the method of introducing innovations into organizations, either by a top-down or bottom-up approach; 6) the plan should have well-defined priorities and objectives despite its flexibility as projects evolve and grow over time.     This report presents a science strategy for the third decade of the National Water-Quality Assessment (NAWQA) Program, which since 1991, has been responsible for providing nationally consistent information on the quality of the Nation's streams and groundwater; how water quality is changing over time; and the major natural and human factors that affect current water quality conditions and trends. The strategy is based on an extensive evaluation of the accomplishments of NAWQA over its first two decades, the current status of water-quality monitoring activities by USGS and its partners, and an updated analysis of stakeholder priorities. The plan is designed to address priority issues and national needs identified by NAWQA stakeholders and the National Research Council (2012) irrespective of budget constraints. This plan describes four major goals for the third decade (Cycle 3), the approaches for monitoring, modeling, and scientific studies, key partnerships required to achieve these goals, and products and outcomes that will result from planned assessment activities. The science plan for 2013–2023 is a comprehensive approach to meet stakeholder priorities for: (1) rebuilding NAWQA monitoring networks for streams, rivers, and groundwater, and (2) upgrading models used to extrapolate and forecast changes in water-quality and stream ecosystem condition in response to changing climate and land use. The Cycle 3 plan continues approaches that have been central to the Program’s long-term success, but adjusts monitoring intensities and study designs to address critical information needs and identified data gaps.     All well-known quality management (QM) models in health care also address issues of the quality of leadership, even if they do so with varying intensity. Skills in managing organisations are a basically important quality in a top manager, but skills in leading people have been revealed as being most relevant. All the manuals of the most common quality management systems specify 'To-Do' lists and tasks for the top management--EFQM and KTQ/proCum Cert have definitively linked them with quality management. Quality needs to be assessed by the degree to which management goals are achieved. However, there is no easy approach to measure leadership abilities and explain it to the public.     Single-Shell Tank Waste Management Area U (WMA U) is in the 200 West Area on the Hanford Site. The area includes the U Tank Farm that contains 16 underground, single-shell tanks and their ancillary equipment and waste systems. WMA U is regulated under the Resource Conservation and Recovery Act of 1976 (RCRA) as codified in 40 CFR Part 265, Subpart F and Washington's Hazardous Waste Management Act (HWMA, RCW 70.105) and its implementing requirements in the Washington State dangerous waste regulations (WAC 173-303-400). Releases of hazardous wastes from WMA U have contaminated groundwater beneath the area. Therefore, the WMA U is being assessed to determine the rate of movement and extent of the contamination released and to determine the concentrations in groundwater. The original finding of groundwater impact was determined from elevated specific conductance in downgradient well 299-W19-41. The elevated specific conductance was attributed to the nonhazardous constituents calcium, magnesium, sulfate, and chloride. Tank waste constituents nitrate and technetium-99 are also present as co-contaminants and have increased over the past several years; however, at concentrations well below the respective drinking water standards. Chromium concentrations in downgradient wells have generally exceeded background levels, but similar levels were also observed in upgradient well 299-W18-25 in early 2000 before it went dry. The objective of this report is to present the current conceptual model for how and where contaminant releases have reached the water table and how that contamination has dispersed in the groundwater system. These efforts will achieve the requirements of a groundwater quality assessment under RCRA [40 CFR 265.93 (d)(4)]. On that basis, a monitoring schedule with appropriate analytes and proposals for new wells and tests are presented in this document.

    During the last decades, socioeconomic change in the catchment of the Spree River, a tributary of the Elbe, has been to a large extent associated with lignite mining activities and the rapid decrease of these activities in the 1990s. There are multiple interconnections between lignite mining and water management both in terms of water quantity and quality. During the active mining period a large-scale groundwater depression cone has been formed while river discharges have been artificially increased. Now, the decommissioned opencast mines are being transformed into Europe's largest man-made lake district. However, acid mine drainage causes low pH in post mining lakes and high concentrations of iron and sulphate in post mining lakes and the river system. Next to potential changes in mining activities, also the potential impacts of climate change (increasing temperature and decreasing precipitation) on water resources of the region are of major interest. The fundamental question is to what extent problems in terms of water quantity and water quality are exacerbated and whether they can be mitigated by adaptation measures. In consequence, long term water resource planning in the region has to formulate adaptation measures to climate change and socioeconomic change in terms of mining activities which consider both, water quantity and water quality aspects. To assess potential impacts of climate and socioeconomic change on water quantity and water quality of the Spree River catchment up to the Spremberg reservoir in the scenario period up to 2052, we used a model chain which consists of (i) the regional climate model STAR (scenarios with a further increase in temperature of 0 and 2 K), (ii) mining scenarios (mining discharges, cooling water consumption of thermal power plants), (iii) the ecohydrological model SWIM (natural water balance), (iv) the long term water management model WBalMo (managed discharges, withdrawal of water users, reservoir operation) and (v) the

The mean score assigned to the whole questionnaire of investigating the viewpoints was 37.5 out of 50. The mean score of the interns’ questionnaire was significantly more than the mean score of the assistants. Discussion: The results obtained indicated that the greatest existing consensus about this plan was the positive impact on the learning of interns in the emergency setting. Thus, it will not only increase the number of patients who the interns are managing during the internship course, but also increases the balance of their workload and they can learn and manage the emergency patients with more leisure.     The growing importance of urban allotments in planning and managing urban areas is due to the combined positive effects on ecosystem services, the economy and human well-being, especially of groups of the urban population that can be vulnerable (e.g. the elderly, immigrants, low-income families). Some studies have highlighted the potential risk of contamination by metals of vegetables grown in urban areas and the lack of appropriate site-specific risk assessments. However, surveys are still lacking on the possibilities of using urban soil as a good substrate to produce vegetables for home consumption. We assessed the soil quality in two areas in Pisa (Italy), one intended for urban horticulture and the other already cultivated for that purpose. We analysed the soils for the main chemical and physical characteristics (texture, bulk density, water stability index, pH, cation exchange capacity, organic carbon, total nitrogen, phosphorous) and elements (Pb, Cu, Ni, Cr, Zn, Cd, As, K, Al and Mn). Our results showed that both areas had physical and chemical heterogeneity due to the effects of urbanization and to the different cultivation techniques employed. The metal content was lower than the guidelines limits, and the soil conditions (pH = 8) greatly reduced the metal mobility. Copper concentration in some of the cultivated area samples was higher than the limits, representing a possible stress factor for the microbial biodiversity and fauna. Our findings demonstrate that site-specific surveys are necessary before planning urban cultivation areas, and educating urban gardeners regarding sustainable cultivation techniques is a priority for a safe environment.     Pacific Northwest National Laboratory conducted a first determination groundwater quality assessment at the Hanford Site. This work was performed for the US Department of Energy, Richland Operations Office, in accordance with the Federal Facility Compliance Agreement during the time period 1996--1998. The purpose of the assessment was to determine if waste from the Single-Shell Tank (SST) Waste Management Area (WMA) B-BX-BY had entered the groundwater at levels above the drinking water standards (DWS). The resulting assessment report documented evidence demonstrating that waste from the WMA has, most likely, impacted groundwater quality. Based on 40 CFR 265.93 [d] paragraph (7), the owner-operator must continue to make the minimum required determinations of contaminant level and of rate/extent of migrations on a quarterly basis until final facility closure. These continued determinations are required because the groundwater quality assessment was implemented prior to final closure of the facility.

    Executive Order 13423, Strengthening Federal Environmental, Energy, and Transportation Management establishes the policy that Federal agencies conduct their environmental, transportation, and energy-related activities in a manner that is environmentally, economically and fiscally sound, integrated, continually improving, efficient, and sustainable. The Department of Energy (DOE) has approved DOE Order 450.1A, Environmental Protection Program and DOE Order 430.2B, Departmental Energy, Renewable Energy and Transportation Management as the means of achieving the provisions of this Executive Order. DOE Order 450.1A mandates the development of Environmental Management Systems (EMS) to implement sustainable environmental stewardship practices that: (1) Protect the air, water, land, and other natural and cultural resources potentially impacted by facility operations; (2) Meet or exceed applicable environmental, public health, and resource protection laws and regulations; and (3) Implement cost-effective business practices. In addition, the DOE Order 450.1A mandates that the EMS must be integrated with a facility's Integrated Safety Management System (ISMS) established pursuant to DOE P 450.4, 'Safety Management System Policy'.     Objective Using quality improvement methodology, our goal was to develop and implement individualized home pain management plans (HPMP) that included pharmacologic as well as non-pharmacologic strategies for children with sickle cell disease (SCD). We hypothesized that successfully implemented HPMPs would have an impact on Emergency Department (ED) use, decreasing ED visits for uncomplicated SCD pain episodes. Methods A multidisciplinary quality improvement team developed a questionnaire to assess the frequency, location and severity of a patient’s pain during a routine, comprehensive visit in order to help the patient and family develop an effective pain management strategy using both pharmacologic and non-pharmacologic actions. Using plan do study act cycles (PDSAs), this team was able to build this process into the daily workflow for all SCD patients age 5 years to 21 years of age. Patients with comprehensive visits scheduled from January 2012 to May 2013 were included (N=188) in the intervention. Results By May of 2013, 88% of eligible patients had an individualized HPMP in place. There was a concomitant reduction in the percentage of SCD patients seen in the ED for uncomplicated SCD pain (6.9% vs. 1.1%). Conclusions Using quality improvement methods, an individualized HPMP intervention was incorporated successfully into the daily workflow of a busy outpatient SCD clinic. This intervention has the potential to improve patient outcomes by decreasing avoidable ED visits as well as reducing overall healthcare costs. PMID:25258504

This paper presents an index that attempts to perform a comparative assessment of groundwater quality across basins controlled by the same or different factors, subject to the same or different pressures and characterized by different availability of water quality measurements spread over the same or different time periods. It serves as an easy to implement and unbiased approach to identify water quality controlling factors.     The quality management of an organization can be defined as the ability to identify, plan and implement programs of measure, analysis, verification and control that allow to monitor management, resources, activities, processes and output/outcome of the same organization, including the satisfaction of the customers. Whatever the model used, it is demonstrated that the management-quality system, either for professional quality or for organization, turns out to be effective even in the health organizations within and to any level of organizational-structural complexity. The present paper concerns the experience of the Italian Society of Cardiovascular Echography (SIEC) on quality certification, both as a scientific society compared to other health organizations and to cardiovascular echo laboratories, and the definition of minimum requirements for the accreditation of the same laboratories. The model most frequently used for quality management is represented by the ISO 9000: Vision 2000, that is a management model with specific reference to the organization and the customer satisfaction. The model applied to the health structure needs a rapid change in mentality that addresses the operators to define, share and achieve objectives to be brought on by means of an active collaboration, group activity and deep sense of belonging necessary to the attainment of expected objectives. When the model is applied by a scientific society, it is necessary to take into account the different structural and functional organization, the constitution and the operators differing on the point of view of origin, experiences, mentality, and roles. The ISO 9000: Vision 2000 model can be applied also to the cardiovascular echo laboratory which may be compared to a simple organization; for its corrected functioning, SIEC has defined minimal requirements for the accreditation, realization and modalities to carry out and manage quality.     As a result of rapid economic growth in the Pingtung Plain, the use of groundwater resources has changed dramatically. The groundwater is quite rich in the Pingtung plain and the most important water sources. During the several decades, a substantial amount of groundwater has been pumped for the drinking, irrigation and aquaculture water supplies. However, because the sustainable use concept of groundwater resources is lack, excessive pumping of groundwater causes the occurrence of serious land subsidence and sea water intrusion. Thus, the management and conservation of groundwater resources in the Pingtung plain are considerably critical. This study aims to assess the conjunct use effect of groundwater and surface water in the Pingtung plain on recharge by reducing the amount of groundwater extraction. The groundwater quality variability and groundwater flow models are combined to spatially analyze potential zones of groundwater used for multi-purpose in the Pingtung Plain. First, multivariate indicator kriging (MVIK) is used to analyze spatial variability of groundwater quality based on drinking, aquaculture and irrigation water quality standards, and probabilistically delineate suitable zones in the study area. Then, the groundwater flow model, Processing MODFLOW (PMWIN), is adopted to simulate groundwater flow. The groundwater flow model must be conducted by the calibration and verification processes, and the regional groundwater recovery is discussed when specified water rights are replaced by surface water in the Pingtung plain. Finally, the most suitable zones of reducing groundwater use are determined for multi-purpose according to combining groundwater quality and quantity. The study results can establish a sound and low-impact management plan of groundwater resources utilization for the multi-purpose groundwater use, and prevent decreasing ground water tables, and the occurrence of land subsidence and sea water intrusion in the Pingtung plain.

    There are numerous documented cases of extraction of fluids from the ground causing surface subsidence. The cases include groundwater, oil and gas, as well as geothermal fluid withdrawal. A recent comprehensive review of all types of man-induced land subsidence was published by the Geological Survey of America. At the early stages of a geothermal power development project it is standard practice in most countries for an environmental impact report to be required. The possibility of geothermal subsidence has to be addressed, and usually it falls on the geophysicists and/or geologists to make some predictions. The advice given is vital for planning the power plant location and the borefield pipe and drain layout. It is not so much the vertical settlement that occurs with subsidence but the accompanying horizontal ground strains that can do the most damage to any man-made structure.

    The objective of the Solid Waste Management Program Plan (SWMPP) is to provide a summary level comprehensive approach for the storage, treatment, and disposal of current and future solid waste received at the Hanford Site (from onsite and offsite generators) in a manner compliant with current and evolving regulations and orders (federal, state, and Westinghouse Hanford Company (Westinghouse Hanford)). The Plan also presents activities required for disposal of selected wastes currently in retrievable storage. The SWMPP provides a central focus for the description and control of cost, scope, and schedule of Hanford Site solid waste activities, and provides a vehicle for ready communication of the scope of those activities to onsite and offsite organizations. This Plan represents the most complete description available of Hanford Site Solid Waste Management (SWM) activities and the interfaces between those activities. It will be updated annually to reflect changes in plans due to evolving regulatory requirements and/or the SWM mission. 8 refs., 9 figs., 4 tabs.

  8. Improving Plan Quality in SAT-Based Planning

    Planning as Satisfiability (SAT) is the best approach for optimally (wrt makespan) solving classical planning problems. SAT-based planners, like SATPLAN, can thus return plans having minimal makespan guaranteed. However, the returned plan does not take into account plan quality issues introduced in the last two International Planning Competitions (IPCs): such issues include minimal-actions plans and plans with "soft" goals, where a metric has to be optimized over actions/goals. Recently, an approach to address such issues has been presented, in the framework of planning as satisfiability with preferences: by modifying the heuristic of the underlying SAT solver, the related system (called SATPLAN(P)) is guaranteed to return plans with minimal number of actions, or with maximal number of soft goals satisfied. But, besides such feature, it is well-known that introducing ordering in SAT heuristics can lead to significant degradation in performances. In this paper, we present a generate-and-test approach to tackle the problem of dealing with such optimization issues: without imposing any ordering, a (candidate optimal) plan is first generated, and then a constraint is added imposing that the new plan (if any) has to be "better" than the last computed, i.e., the plan quality is increased at each iteration. We implemented this idea in SATPLAN, and compared the resulting systems wrt SATPLAN(P) and SGPlan on planning problems coming from IPCs. The analysis shows performance benefits for the new approach, in particular on planning problems with many preferences.

    Quality management has become one of the most important and most debated topics within the service sector. This is especially true for health care, as the controversy rages on how the existing American system should be restructured. Health care reform aimed at reducing costs and ensuring access to all Americans cannot be allowed to jeopardize the quality of care. As such, total quality management (TQM) has become a vital ingredient to strategic planning within the health care domain. At the heart of any such quality improvement effort is the issue of measurement. TQM cannot be effectively utilized as a competitive weapon unless quality can be accurately defined, measured, evaluated, and monitored over time. Through such analysis a hospital can elect how to expend its limited resources toward those quality improvement projects which will impact customer perceptions of service quality the most. Thus, the purpose of this report is to establish a framework by which to approach the issue of quality measurement, delineate the various components of quality that exist in health care, and explore how these elements affect one another. We propose that the issue of quality measurement in health care be approached as an integration of service quality attributes common to other service organizations and technical quality attributes unique to health care. We hope that this research will serve as a first step toward the synthesis of the various quality attributes inherent in the health care domain and encourage other researchers to address the interactions of the various quality attributes.     With the rapid movement of both individuals and groups away from fee-for-service health care into managed care, concerns have been expressed appropriately that the quality of care may be affected adversely. Over the past several years, a number of developments have taken place to respond to these concerns. This quality movement in managed care has not been without some issues and considerations, however. This article first describes the National Committee for Quality Assurance (NCQA) and the prominent role it has played in this movement. Next, quality improvement study design is addressed in the context of assuring quality, controlling costs, and achieving NCQA accreditation. The effect that capitation, as a payment strategy for providers, has on data quality is then described. Fourth, the value of partnering is explored.     The National Maintenance Point (NMP) Branch of the US Army Industrial Operations Command (IOC) serves as the Single Manager for Conventional Ammunition (SMCA) agent for managing the renovation, modification, recycling, and disposal of conventional ammunition, thereby improving readiness. The mission of the NMP includes program management for demilitarization (demil) activities, ammunition maintenance, and ammunition peculiar equipment (APE) projects. Through an Interagency Agreement between the US Army and the US Department of Energy, Argonne National Laboratory (ANL) is developing an integrated data management system, called the Demil Planning and Management System (DPMS), for IOC. DPMS is intended to help NMP efficiently manage information on ongoing demil project activities and asset inventories, plan future projects, and allocate budgets. This system, when fully implemented, will also make it possible for the user community to interactively access the DPMS database; perform data entry and queries; and run reports through network, modem, and Internet access to the system. This paper describes the principal components of the DPMS, current capabilities, and planned enhancements.

    Degradation of air quality in Asia due to economic growth during Past two decades is evident. The World Bank with its new mandate for sustainable development is working with Asian Governments to reverse the trend. This paper briefly describes initiatives called URBAIR and RAINS-ASIA. Urban Air Quality Strategy in Asia (URBAIR) program seeks to assist governments and the private sector in developing tools to identify immediate and long-term actions to restore air quality in Asian cities. The URBAIR program, conducted for four selected cities (Bombay, Jakarta, Kathmandu, Manila). Since air pollution management in Asian cities is in a preliminary stage, there is need to develop a comprehensive Action Plan as a framework for preparing the Air Quality Management Strategy in each city. In preparing this Action Plan, URBAIR has identified the main aspects of air pollution in the cities. These include major pollutants, their sources, and their health and economic impacts. Air quality models have been evaluated and adapted to the data available for these cities, and monitoring needs identified. An Action Plan has been proposed by the city based working groups for a comprehensive Air Quality Management Strategy.

    Before we start analysing the philosophy of Total Quality Management it is worthwhile going back to the early days of quality and the quality movement. In fact, the quality concept dates back to the creation of Adam and Eve: “And God saw every thing that he had made, and, behold, it was very good”. (Genesis A 31). It is remarkable that at the end of each day, looking at his creations God was saying, “This is good”. However, at the end of the sixth day, after he finished the creation of human beings, he said, “This is very good”. It is amazing that he did not say, “This is excellent”. This is because excellence is something that we gain after tireless effort. God left room for improvement in order to challenge us and make our life more attractive, which has really happened!

    The rapid growth of new forms of managed care in the United States in recent decades has brought with it increasing concerns regarding the quality of care delivered by practitioners in these plans. This article examines the various regulatory demands that are being placed on Managed Care Organizations (MCOs). The authors look at the major determinants that are likely to bring about significant changes in the health care sector for both patients and providers and predict how these shifts will affect the quality of health care services in the near future. They discuss how the quality of health care, rather than the cost of those services, can become and remain the primary factor in the delivery of health care services. Ultimately, they conclude that increased participation by the federal government is required to protect the rights of patients and ensure better quality and accountability for health care services delivered by MCOs.     Risk management is the systematic application of management policies, procedures, and practices to the tasks of analyzing, evaluating, controlling and monitoring risk (the effect of uncertainty on objectives). Clinical laboratories conduct a number of activities that could be considered risk management including verification of performance of new tests, troubleshooting instrument problems and responding to physician complaints. Development of a quality control plan for a laboratory test requires a process map of the testing process with consideration for weak steps in the preanalytic, analytic and postanalytic phases of testing where there is an increased probability of errors. Control processes that either prevent or improve the detection of errors can be implemented at these weak points in the testing process to enhance the overall quality of the test result. This manuscript is based on a presentation at the 2nd International Symposium on Point of Care Testing held at King Faisal Specialist Hospital in Riyadh, Saudi Arabia on October 12-13, 2010. Risk management principles will be reviewed and progress towards adopting a new Clinical and Laboratory Standards Institute Guideline for developing laboratory quality control plans based on risk management will be discussed. Successful implementation of the inspections depends upon planning for verification activities such as comparisons of records and reports, measurements of material inventories, and employment of containment/surveillance methods.     Good specification of a product’s performance requires adequate characterization of relevant properties. Particulate products are usually characterized by some PSD, shape or porosity parameter(s). For proper characterization, adequate sampling, dispersion, and measurement procedures should be available or developed and skilful personnel should use appropriate, well-calibrated/qualified equipment. The characterization should be executed, in agreement with customers, in a wellorganized laboratory. All related aspects should be laid down in a quality handbook. The laboratory should provide proof for its capability to perform the characterization of stated products and/or reference materials within stated confidence limits. This can be done either by internal validation and audits or by external GLP accreditation.

    Planning for quality assurance (QA) in research and development (R D) is like stealing eggs without waking up the chickens. The QA program should be as unobtrusive as possible. Researchers require a QA program that affords them an environment capable of supporting repeatable experiments with accurate data without unduly stifling their creative abilities. Careful advance planning ensures that the intensity of control provided by quality-related systems is commensurate with the importance and scope of the activities being performed. Good scientific practices applied to small bench-scale projects may require minimal additional controls. As projects increase in size and complexity the controls imposed through planning must, by necessity, be increased. Research and development QA planning, just like any other planning, involves all affected individuals. The application of control systems is determined by factors such as customer or sponsor requirements, the importance of an item or activity to the experiment's success, and the organizational complexity of the project. Many larger experiments are highly dependent on quality-related support activities such as calibration, engineering design, and inspection provided by organizations outside the R D group. Since, in most cases, the expense of support activities is taken directly from funds available for research, it is important for the researchers to be involved in the planning efforts to help determine and agree with the level of QA effort required. A single plan will often suffice for organizations engaged in large numbers of similar experiments. Complex experiments may require unique QA plans or additions to existing plans. Once implemented, the R D QA plans, like any others, require audits or surveillances and may require revisions if the scope of the experiment changes. 1 ref., 1 fig.

    Background Skilled managers are an important component of quality improvement (QI) infrastructure, but there has been little evaluation of QI infrastructure, which is needed to guide enhancement of this capacity. Methods Quality managers at 97 acute care hospitals in Ontario, Canada, were surveyed by mail to describe how their roles were integrated with QI performance objectives. Binary and scaled responses were analysed quantitatively, and open-ended responses were analysed thematically. Results The response rate was 79.4%. Many QI managers were new to their role and had no support staff despite responsibility for multiple portfolios. Respondents thought that QI objectives should be less reactive to hospital executives or boards, adverse events or demands from government and accreditation bodies, and recommended that dedicated QI managers proactively apply explicit strategic plans and engage executives and clinicians. Findings were consistent regardless of rank, staffing or hospital type. Those with master's training and greater experience were more involved in strategic planning, data analysis and communication. Conclusions QI is not well resourced in most acute care hospitals in Ontario. To develop QI capacity, investment and QI training may be required. Research should empirically establish objective performance measures of QI capacity to guide investment and evaluation. PMID:20172879

    Appropriate deployment of technological innovation contributes to improvement in the quality of healthcare delivered, the containment of cost, and access to the healthcare system. Hospitals have been allocating a significant portion of their resources to procuring and managing capital assets; they are continuously faced with demands for new medical equipment and are asked to manage existing inventory for which they are not well prepared. To objectively manage their investment, hospitals are developing medical technology management programs that need pertinent information and planning methodology for integrating new equipment into existing operations as well as for optimizing costs of ownership of all equipment. Clinical engineers can identify technological solutions based on the matching of new medical equipment with hospital's objectives. They can review their institution's overall technological position, determine strengths and weaknesses, develop equipment-selection criteria, supervise installations, train users and monitor post procurement performance to assure meeting of goals. This program, together with cost accounting analysis, will objectively guide the capital assets decision-making process. Cost accounting analysis is a multivariate function that includes determining the amount, based upon a strategic plan and financial resources, of funding to be allocated annually for medical equipment acquisition and replacement. Often this function works closely with clinical engineering to establish equipment useful life and prioritization of acquisition, upgrade, and replacement of inventory within budget confines and without conducting time consuming, individual financial capital project evaluations.     In summary, planning a case management system involves moving through sequenced stages; namely, (1) a developmental stage; (2) a phase-in stage; and (3) an operational stage. During the developmental stage, before processes have been formalized, attention is given to the political realities, the formation of a representative planning group, formulating goals and objectives, obtaining administrative support, information gathering, assessment of needs, resource procurement, program structure and design, selection of a case management model, participatory decision making, determining organizational fit, and beginning networking. During the phase-in or early implementation state, the formal stage of system introduction, attention is given to the establishment of interorganizational relationships, contracting for services, job descriptions, work assignments, training, problem solving, and conflict resolution. During the operational or full implementation stage, a period when the system should become more stabilized, attention needs to be given to managing movement of the client through the system, the flow of information, program updating, quality assurance, recordkeeping, resource management, evaluating, and system refurbishing. In practice, these stages will interact and overlap. Closure, if the system is to remain viable and open to change, should never occur. PMID:10110505

The dental component includes 24 dental clinics and a network of 2,500 dentists in a PPO plan, supported by a quality management team. An important feature of this network of clinics and dentists is the opportunity for pooling and analyzing data on oral health- care outcomes. These data are used to mentor the entire office team, to drive systemwide improvements in treatment protocols, and as part of providers' compensation.     Transparency through public reporting of quality data is key to achieving the Institute of Medicine's (IOM) vision for 21st century health care. This article reviews the status of States' voluntary public reporting of Medicaid managed care (MMC) quality data, and analyzes these data. Twenty-one States, including 17 of the 20 largest managed care States, have made plan-level data publicly available online, although the data are sometimes thin, with few measures reported, hard-to-access, and old. We conclude that CMS could better leverage the power of public reporting for quality improvement (QI) by increasing the visibility of health plan employer data and information set (HEDIS®) data that States already collect.     Establishment of quality management system (QMS) plays a critical role in the clinical data management (CDM). The objectives of CDM are to ensure the quality and integrity of the trial data. Thus, every stage or element that may impact the quality outcomes of clinical studies should be in the controlled manner, which is referred to the full life cycle of CDM associated with the data collection, handling and statistical analysis of trial data. Based on the QMS, this paper provides consensus on how to develop a compliant clinical data management plan (CDMP). According to the essential requirements of the CDM, the CDMP should encompass each process of data collection, data capture and cleaning, medical coding, data verification and reconciliation, database monitoring and management, external data transmission and integration, data documentation and data quality assurance and so on. Creating and following up data management plan in each designed data management steps, dynamically record systems used, actions taken, parties involved will build and confirm regulated data management processes, standard operational procedures and effective quality metrics in all data management activities.     The Software Cost and Quality Engineering methodology developed over the last two decades at IBM Federal Sector Div. is used to plan the NASA Space Station Data Management System (DMS). An ongoing project to capture this methodology, which is built on a foundation of experiences and lessons learned, has resulted in the development of a PC-based tool that integrates cost and quality forecasting methodologies and data in a consistent manner. This tool, Software Cost and Quality Engineering Starter Set (SCQESS), is being used to assist in the DMS costing exercises.
