  1. [Prevention rather than cure: the emergence and first stage of the Centros de Higiene Infantil in Mexico City, 1922-1932].

    This article deals with the main features of the emergence and first ten years of the Centros de Higiene Infantil, facilities run by the Departamento de Salubridad Pública from 1922 on in Mexico City with the goal of providing care for mothers from pregnancy onwards and children from birth to two years of age. It reviews the actions that gave rise to this project and how it became established. It analyzes the structure of these centers, the characteristics of the mothers and children seen there and the functions performed by doctors and nurses, stressing the notion of preventing childhood illnesses, and ends with a first assessment of the effects and limitations of these centers.

    Created in the early 1920s, at a moment when the country's psychiatric field was embracing the preventive outlook, the Liga Brasileira de Higiene Mental included within its members the elite of Brazilian psychiatry, along with a number of physicians and intellectuals. The article discusses the institution's proposals for intervention among children. The league ended up incorporating into its theoretical arsenal the basic themes of mental hygiene and eugenics as part of its general goal of collaborating in Brazil's process of "racial sanitation". With this objective in mind, and viewing the child as a "pre-citizen" who is a "fundamental part within the man of the future", league members included the children's issue in their projects and saw an imperative need for mental health care from early ages on.

    The superficial mycoses are very common infectious diseases and therefore are a frequent reason for medical consultation. The aim of this study was to determine the diagnostic frequency of superficial mycoses in the Mycology Department of the Instituto Nacional de Higiene "Rafael Rangel" during 14 years (2001-2014). A retrospective cross-sectional study was performed to review the mycological records of patients with presumptive diagnosis of superficial mycosis. Nails, hairs and epidermal scales were the processed samples. The identification of fungi was performed by macro and microscopic observation of colonies and biochemical and physiological tests, as required of the isolated agent. For the investigation of Malassezia spp. only direct examination was performed. Of the 3 228 samples processed, 1 098 (34%) were positive and their distribution according to the etiological agent was: dermatophytes 79.5%; 10.9% yeasts; non-dermatophytes fungi 5.1% and 4.5% Malassezia spp. The most frequently isolated dermatophyte was Trichophyton rubrum Complex (70.1%), followed by T mentagrophytes complex (15.1%), Microsporum canis (9.4%) and Epidermophyton floccosum (4%). The most frequent ringworms Were: Tinea unguium (66.8%), followed by Tineapedis (16.4%) and Tinea capitis (8.1%). Candida parapsilosis complex (37.5%) was the most frequently isolated yeast and Fusarium spp. (53.6%) was the most isolated among non-dermatophyte fungi, followed by Aspergillus spp. (19.6%) and Acremonium spp. (10.7%). The identification of the etiological agent is essential to guide appropriate treatment. This study constitutes an important contribution to the knowledge of the epidemiology of superficial mycoses in our country.

  6. Self-perception of oral health in older adults from an urban population in Lisbon, Portugal.

    To analyze if the self-perception of oral health in the urban context is associated with sociodemographic factors that interfere in the life quality of oral health. Cross-sectional study with convenience sample of older individuals (65 years old or more) enrolled in the Agrupamento de Centros de Saúde de Lisboa Norte (ACES Lisboa Norte - Health Centers Groupings North Lisbon). The self-perception of oral health and associated life quality was evaluated by the Geriatric Oral Health Assessment Index and the individuals were classified according to sociodemographic characteristics. The internal consistency of the questionnaire was evaluated by Cronbach's alpha (α). Later, we used binary logistic regression models to characterize the factors associated with the self-perception of oral health, considering the sociodemographic variables and the older adults' clinical conditions of oral health and establishing the crude and adjusted (to age) odds ratios and their 90% confidence intervals. A total of 369 older adults participated in this study, with an average age of 74.2 years (SD = 6.75); 62.9% were female. On average, the index was moderated, with tendency to be high: 32.9 (SD = 3.6; 12-36 interval). The Cronbach's alpha was high: 0.805. Age, marital status, and the last dental appointment were the factors significantly associated with self-perception of oral health. The study shows that these individuals have a moderate, with tendency to high, self-perception of oral health. The self-perception of oral health assessment allowed us to identify the main associated sociodemographic factors. This instrument can help guiding planning strategies and oral health promotion directed toward a better life quality for this population group.     Introducción: estimar la prevalencia del tabaquismo y analizar cómo se diagnostican y se trata a los fumadores diagnosticados de EPOC.Métodos: estudio epidemiológico, transversal, multicéntrico (30 centros salud de la provincia de León). Incluyó pacientes mayores de 35 años diagnosticados y tratados de EPOC. Variables analizadas: edad, sexo, hábitat, datos antropométricos, tabaquismo, número de paquetes/año, cooximetría, dependencia (escala analógico-visual), motivación (test de Fagerström), autoeficacia, estado anímico, intentos previos, terapia cognitivo-conductual, tratamiento farmacológico (TSN, bupropión, vareniclina) y recaídas. Los resultados se expresan con sus IC al 95,5%.Resultados: se incluyó a 833 pacientes, el 85,8% varones, edad media: 64,69 (53,66-75,61) años y 20,65 (4,47-36,8) años de evolución de la EPOC. El 86,67% (80,30-93,30) tenían antecedentes de tabaquismo (n = 722), de 35,26 (17,87-52,64) años de evolución, con consumo medio 28,36 (9,60-46,86) paquetes año, p < 0,001, siendo el 58% fumadores severos. El 57,4% (53,90-60,60) son exfumadores. El 29,3% (26,40-32,70) fumadores activos declarados vs. 35,11% (33,90-37,12) fumadores diagnosticados por cooximetría p < 0,05. Los 288 fumadores activos, presentaban baja motivación (49,80%), alta dependencia (49,5%), actitud negativa (52,60%), bajo estado de ánimo (32,05%), con 2,72 (1,74-3,67) intentos para dejar de fumar, p < 0,0001. La terapia conductivo-conductual (TCC) combinado con tratamiento farmacológico se realizó en el 55,8% (52,2-54,9), p < 0,05; La intervención más efectiva fue TCC combinada con vareniclina logrando una abstinencia del 29,86%. En total dejaron de fumar un 51,05% (49,49-52,70) de los pacientes con EPOC, p < 0,001.Conclusiones: la prevalencia de tabaquismo en la EPOC en nuestro medio continúa siendo inadmisiblemente elevada. Es necesaria una mayor implicación para disminuir su impacto en la salud de estos pacientes.

    Based on historical references of scientific communication, we analyzed the issues of the Boletim do Instituto de Higiene de São Paulo and of the Arquivos da Faculdade de Higiene e Saúde Pública da Universidade de São Paulo. Published respectively from 1919 to 1946 and from 1947 to 1966, they totaled 120 issues. In their 48 years of publication, their goal was to disseminate the scientific production of the institution and to legitimize the theoretical debate of the field, in addition to supporting the public health intervention models, written by leading researchers of the institution and by contributors and managers in the field of public health. Both the Boletim and the Arquivos were recognized as scientific communication of national reference, and have laid the foundations for the creation of the Revista de Saúde Pública, in 1967. RESUMO Com base em referenciais históricos da comunicação científica, foram analisadas as edições do Boletim do Instituto de Higiene de São Paulo e dos Arquivos da Faculdade de Higiene e Saúde Pública da Universidade de São Paulo. Publicadas respectivamente de 1919 a 1946 e de 1947 a 1966, totalizam 120 edições. Em seus 48 anos de publicação, o objetivo era disseminar a produção científica da instituição e legitimar o debate teórico do campo, além de apoiar os modelos de intervenção de saúde pública, escrita por importantes pesquisadores da instituição e colaboradores e gestores do campo da saúde pública. Tanto o Boletim como os Arquivos foram reconhecidos como comunicação científica de referência nacional e lançaram as bases para a criação da Revista de Saúde Pública, em 1967.

Fueron utilizados los modelos de regresión de Poisson para identificar diferencias entre los sexos en la prevalencia de déficit de autocuidado de los pies y estilo de vida, ajustándolas por características socioeconómicas, clínicas, tabaquismo y alcoholismo. la prevalencia de déficit de autocuidado de los pies, caracterizada por: baja frecuencia de secado de los espacios interdigitales; falta de evaluación periódica de los pies; hábito de andar descalzo; higiene insatisfactoria; y, corte inadecuado de uñas, fue significativamente mayor entre los hombre. Sin embargo, estos presentaron menor prevalencia en la práctica de escaldar los pies y en el uso de calzados inadecuados en comparación a las mujeres. En relación al estilo de vida, los hombres también presentaron comportamientos menos saludables ya que tienen significativamente menor control alimentario y no realizan los exámenes de laboratorio referentes al perfil lipídico, con la frecuencia recomendada. considerar las diferencias de género en el autocuidado de los pies y en el estilo de vida, le permite al equipo de enfermería realizar actividades educacionales e intervenciones en los factores de riesgo para la ulceración de los pies.

    To identify the prevalence of the main oral problems present in special needs children and to relate the underlying conditions with the clinical and demographic variables. The study was based on the physical examination of 47 students from the Associação de Pais e Amigos dos Excepcionais diagnosed as Down syndrome, cerebral palsy and intellectual deficit. For data collection, we used a self-administered questionnaire that included indices of dental caries and oral hygiene, Angle classification, malposition of dental groups and oral hygiene habits. The predominant age group was 12-25 years (46.8%) and most patients were male (55.3%). Regarding daily brushing, 63.8% reported brushing their teeth three times a day, and 85.1% did it by themselves. A total of 48.9% were rated as Angle class I, and 25.5% had no type of malocclusion. A high dental carries index (decayed, missing, filled >10) was observed in 44.7%, and 53.2% had inadequate oral hygiene (zero to 1.16). There was a statistically significant difference between cerebral palsy and the act of the participants brushing their teeth by themselves. There was a high decayed-missing-filled teeth index and malocclusion class I, as well as inadequate oral hygiene. The type of underlying condition of the participants influenced the act of brushing teeth by themselves. Conhecer a prevalência dos principais problemas bucais em crianças com necessidades especiais, e relacionar as doenças de base com variáveis clínicas e demográficas. O estudo foi realizado a partir de exame clínico em 47 alunos da Associação de Pais e Amigos dos Excepcionais com diagnóstico médico de síndrome de Down, paralisia cerebral e deficit intelectual.     In 1986 the Instituto Nacional de Seguridad e Higiene en el Trabajo (INSHT), established the "Programa interlaboratorios de control de calidad de mercurio en orina (PICC-HgU)". The operation of this scheme is explained, criteria for evaluation of laboratory performance are defined and some results obtained are reviewed. Since the scheme started, an improvement in the overall performance of laboratories has been observed. The differences in the analytical methods used by laboratories do not seem to have a clear influence on the results.

    During the Second Reign, the regulation and inspection of food trade in the city of Rio de Janeiro were characterized by heated clashes between three agencies that had overlapping powers in the matter: the Câmara Municipal (Municipal Chambers), the Secretaria de Polícia (Police Department), and the Junta Central de Higiene Pública (Central Public Hygiene Board). The article analyzes the conflicts between these agencies, while taking into account their coincidental duties and power disputes and also underscoring the Chamber's effort to preserve its former public health responsibilities, essential to protecting its governmental capacities.

    SULTATS La prévalence globale des fumeurs actuels est de 28,1%. Près de la moitié des personnes âgées ont fumé à un moment quelconque dans leur vie et parmi celles-ci, 44% ont cessé de fumer. Les personnes âgées ont cessé de fumer lorsqu’elles en ont perçu les effets négatifs sur la santé. Le fait d’avoir au moins une maladie chronique et de souffrir d’une incapacité fonctionnelle ont augmenté d’une façon significative les chances d’arrêt du tabagisme. De plus, le fait d’être un ancien fumeur a augmenté le risque d’admission hospitalière. CONCLUSIONS Cette étude est d’une importance particulière car ses implications s’élargissent à des collectivités à faibles revenus similaires et chez les réfugiés dans la région ainsi qu’ailleurs. Il est nécessaire que les responsables de la santé publique fassent un effort plus concerté pour cibler les personnes âgées comme groupe en vue d’interventions d’arrêt du tabagisme, d’autant plus que les avantages en matière de mortalité et de santé ont aujourd’hui été bien documentés. RESUMEN OBJETIVOS Investigar los tipos de hábito tabáquico en una población de personas ancianas, de bajos ingresos y determinar los factores pronósticos del abandono del hábito. Asimismo, se analizó la importancia del tabaquismo en relación con otros factores de riesgo de hospitalización. DISEÑO Los datos formaban parte de un estudio de salud urbana de 740 personas de ≥60 años de edad, en tres comunidades suburbanas de bajo estrato socioeconómico en Beirut, una de las cuales era un campo de refugiados. Se administró una entrevista estructurada que aportaba amplia información social y sanitaria. RESULTADOS La prevalencia global de fumadores fue de 28,1%. Casi la mitad de las personas ancianas había sido fumadora en algún momento y 44% habían abandonado el tabaquismo.     Objetivos: estimar el porcentaje de pacientes con EPOC adiestrados en la consulta para el manejo de inhaladores en la provincia de León.Métodos: estudio epidemiológico, transversal, multicéntrico (30 centros de salud de la provincia de León). Incluyó pacientes mayores de 35 años diagnosticados y tratados de EPOC. Variables a estudio: edad, sexo, hábitat, datos antropométricos, estado nutricional, tabaquismo, espirometría postboroncodilatadora, disnea (mMRC), reagudizaciones, gravedad (Índice Bodex), hospitalizaciones, tratamiento, seguimiento y caracterización del fenotipo (GesEOPC 2014). Los resultados se expresan con sus IC al 95,5%.Resultados: se incluyeron 833 pacientes, el 85,8% varones, con edad media de 64,69 (53,66-75,61) años y 20,65 (4,47-36,8) años evolución de la EPOC. Empleaban 1,88 (1,64-2,16) dispositivos inhaladores de media, p = 0,006, (57% de forma correcta, 23% regular y el 20% incorrecta). El 20,9% no recibieron adiestramiento para usar inhaladores frente al 79,1% adiestrados, p < 0,001, (9,4% por neumólogos, 20,3% enfermeras y 43,5% médicos familia, p = 0,002). Los pacientes bien adiestrados realizan mejor el tratamiento, en el 60,60% (58,91-62,29), p = 0,002. No hubo diferencias significativas en el adiestramiento por tabaquismo, gravedad, ingresos hospitalarios, ni calidad de vida, obteniendo una reducción significativa del número de agudizaciones, siendo de 1,59 (1,12-2,15) reagudizaciones medias en el grupo adiestrado frente a 3,29 (2,50-4,11) en los no adiestrados, p = 0,002.Conclusiones: el nivel de adiestramiento en el uso de inhaladores en los pacientes con EPOC es insuficiente en nuestro medio. La mejor formación de los profesionales y la simplificación de los dispositivos contribuirán a que un mayor número de pacientes realicen el tratamiento de forma adecuada.

    To analyze the interventions aimed at the practice of sleep hygiene, as well as their applicability and effectiveness in the clinical scenario, so that they may be used by pediatricians and family physicians for parental advice. A search of the PubMed database was performed using the following descriptors: sleep hygiene OR sleep education AND children or school-aged. In the LILACS and SciELO databases, the descriptors in Portuguese were: higiene E sono, educação E sono, educação E sono E crianças, e higiene E sono E infância, with no limitations of the publication period. In total, ten articles were reviewed, in which the main objectives were to analyze the effectiveness of behavioral approaches and sleep hygiene techniques on children's sleep quality and parents' quality of life. The techniques used were one or more of the following: positive routines; controlled comforting and gradual extinction or sleep remodeling; as well as written diaries to monitor children's sleep patterns. All of the approaches yielded positive results. Although behavioral approaches to pediatric sleep hygiene are easy to apply and adhere to, there have been very few studies evaluating the effectiveness of the available techniques. This review demonstrated that these methods are effective in providing sleep hygiene for children, thus reflecting on parents' improved quality of life. It is of utmost importance that pediatricians and family physicians are aware of such methods in order to adequately advise patients and their families.     The article addresses the Amazon Sanitation Plan and the political context in which it was formulated between 1940 and 1941. It examines the role of Getúlio Vargas, the activities of the plan's main protagonists (such as Evandro Chagas, João de Barros Barreto, and Valério Konder), its key proposals, and its demise as of 1942 upon creation of the Special Public Health Service (Sesp), which grew out of cooperation agreements between Brazil and the US following both nations' involvement in World War II. A reproduction of the Plan as published in the Arquivos de Higiene in 1941 is included.

  1. Mid-point for open-ended income category and the effect of equivalence scales on the income-health relationship.

    To estimate the mid-point of an open-ended income category and to assess the impact of two equivalence scales on income-health associations. Data were obtained from the 2010 Brazilian Oral Health Survey (Pesquisa Nacional de Saúde Bucal--SBBrasil 2010). Income was converted from categorical to two continuous variables (per capita and equivalized) for each mid-point. The median mid-point was R$ 14,523.50 and the mean, R$ 24,507.10. When per capita income was applied, 53% of the population were below the poverty line, compared with 15% with equivalized income. The magnitude of income-health associations was similar for continuous income, but categorized equivalized income tended to decrease the strength of association.

    Acellular dermal matrix grafts have become a good alternative to autogenous soft tissue grafts in root coverage. Until now, the literature has reported short- or medium-term data regarding the stability of the gingival margin after the use of acellular dermal matrix on root coverage. The aim of this article is to describe a case report with 10 years of evolution with creeping attachment that developed bucally on a moderate recession of a maxillary canine with an old composite restoration subsequent to an acellular dermal matrix. Long-term creeping attachment and complete root coverage on a restored tooth treated with acellular dermal matrix has not been previously reported in the dental literature.

  3. Determination of radionuclides in environmental test items at CPHR: traceability and uncertainty calculation.

    Information about how the laboratory of Centro de Protección e Higiene de las Radiaciones (CPHR), Cuba establishes its traceability to the International System of Units for the measurement of radionuclides in environmental test items is presented. A comparison among different methodologies of uncertainty calculation, including an analysis of the feasibility of using the Kragten-spreadsheet approach, is shown. In the specific case of the gamma spectrometric assay, the influence of each parameter, and the identification of the major contributor, in the relative difference between the methods of uncertainty calculation (Kragten and partial derivative) is described. The reliability of the uncertainty calculation results reported by the commercial software Gamma 2000 from Silena is analyzed.

  4. [Overlay prosthetic solution in subtotal edentation treatment].

    The preservation of the natural dental roots represents a big advantage for the overdenture prosthetic appliances. The realization of an overdenture needs a perfect correlation of all the clinical and technological factors, in all the prosthetics steps. Another important aspect is represented by the possibility of a correct treatment of the remaining roots. The remaining roots will be correctly endodontically treated. The root section will be prepared with two inclines bucally and orally located. A metallic cup will cover the root section. On the inner surface of the overdenture the dental technician will realize specific preparations corresponding to this cups. This kind of prosthetic treatment is indicated only for patients with a very good oral hygiene and a good general health.

  5. Impact of dental orientation given to mothers during pregnancy on oral health of their children.

    To analyze the perception of mothers about oral health of their children, as well as to check the influence of demographic variables, perception and preventive practice in oral health of mothers regarding guidance received during pregnancy. Quantitative and cross-sectional field study, with a non-probability sample formed by all mothers who attended the primary healthcare unit of Ijuí (RS), Brazil, from January to July 2014, comprising a sample of 79 women. Self-applied questionnaires were given to these mothers. Data analysis was carried out using descriptive and inferential statistics, the χ2 test at a significance level of 5%. The mothers who received dental orientation during pregnancy had greater perception of oral health of their children. The mean age of mothers was 26 years, most of them attended high school education (32.9%) and worked outside the home (60.8%). There was a statistically significant relation between the outcome variable, dental orientation during pregnancy, and the independent variables: schooling level of mothers, occupation, baby's first visit to the dentist, duration of breastfeeding, beginning of baby's tooth brushing and knowledge about dental decay (p<0.005). Mothers with higher schooling levels and who worked outside the home had more knowledge about oral care, because they received dental orientation during pregnancy. The dental guidance during pregnancy influences the mother in the procedures adopted with their children, as to early oral hygiene, first dentist appointment, duration of breastfeeding, knowledge about the factors that lead to dental decay.     para implantação e os custos operacionais de uma Equipe de Saúde Bucal modalidade tipo I na Estratégia Saúde da Família. Estudo de avaliação econômica, tipo análise de investimentos e custos operacionais de uma equipe de saúde bucal no município de Salvador, Bahia, Brasil. O cálculo dos investimentos para implantação foi obtido pela soma dos investimentos em obras civis e instalações em rateio, equipamentos, móveis e instrumentais. Para os custos operacionais, foi analisada a série histórica de 2009 a 2012 e adotou-se o mês de dezembro de 2012 para levantamento dos valores monetários vigentes. Os custos foram classificados em custos diretos variáveis (materiais de consumo) e custos fixos diretos (salários, manutenção, depreciação do capital fixo dos equipamentos, instrumentais, móveis e edificações), além dos custos fixos indiretos (higienização, segurança, energia e água). Foi também calculada a participação do Ministério da Saúde no financiamento e descritos os fatores que influenciam o comportamento dos custos. O investimento para implantação de uma Equipe de Saúde Bucal modalidade tipo I foi de R$29.864,00. Os custos operacionais de uma Equipe de Saúde Bucal modalidade tipo I situaram-se em torno de R$95.434,00 por ano. Os incentivos financeiros do Ministério da Saúde para investimentos cobriram 41,8% dos investimentos com implantação, enquanto o município participou com 59,2% do total. Para os custos operacionais, a participação do Ministério da Saúde foi de 33,1%, enquanto o município participou com 66,9%. Dentro dos custos operacionais, o elemento de maior peso foram os salários, representando 84,7%. Problemas com a regularidade no abastecimento dos insumos e manutenção de equipamentos influenciam sobremaneira na composição dos custos, além de reduzir a oferta de serviços à população-alvo, resultando em provável ineficiência do serviço.     The present investigation had as objective identifying in a family the possible factors of risk related to the use of alcohol and tobacco in the children and adolescents. It is important to emphasize that study of this nature within a social and culture perspective expresses the attempt to include/understand the factors of risk for the use of tobacco and to drink alcoholic the environmental influences in the familiar surroundings views to prevent futures cases with dependency. For the study used a sample of one hundred families, to that applied to an instrument pre to them established with the people in charge of the respective families. As result were obtained 51% of the schooling level are low, 54% has inferior wage to the basic one, 61% to drink alcoholic.     About 50 cases of asbestosis have been descirbed in Spain from 1948 through 1974. Since 1975 the Instituto Territorial de Barcelona, Servicio Social de Higiene y Seguridad del Trabajo, has initiated a survey of all the industries with an asbestosis risk in the Barcelona area. Nearly 300 cases of asbestosis have been detected to date. Given the poor hygienic conditions of most of the industries, with an asbestosis risk, and the considerably large number of exposed people, it can easily be predicted that a rapid increase of the incidence of the disease in the years to come will occur. Most of the observed cases in Barcelona were from two fibrocement industries. Of a total of 1003 workers examined, 247 (about 25%) had asbestosis.

    An ecological evaluation of retrospective and descriptive temporary trend was conducted from 1999 to 2000 .in 8 water bodies of Fomento, province of Sancti Spiritus. To evaluate the effectiveness and permanence of the biolarvicide, there were used data of systematic samplings and of control actions of the provincial surveillance and antivectorial fight programs taken from the records of each breeding place in the Municipal Unit of Higiene and Epidemiology of the locality. Doses of 10 ml of active ingredient per square meter were administered. It was attained the reduction and stabilization of the larval and adult indices in human primer of important species which are vectors of malaria, phyliriasis, and Western Nile's fever. The extension of the larval recovery range up to 3 weeks was proved.

    A 24 year old woman who complained of maculo-papulo rash, genital and bucal aphthous ulcers, abdominal pain, minor dyspnea and visual loss in both eyes. Funduscopy revealed a bilateral occlusive vasculitis including central vessels. Treatment was initiated with a methylprednisolone bolus (1 g/24h) and infliximab 5mg/kg/day (0-2-6 weeks and every 8 weeks). The treatment prescribed induced a fast remission. Visual acuity improved. The patient did not suffer any other relapse after one year of follow-up. An initial treatment with Infliximab should be considered in Behçet disease for serious outbreaks, such as macular occlusive vasculitis with ischemia.     Objetivos: estimar las actividades preventivas que realizan los pacientes diagnosticados de enfermedad pulmonar obstructiva crónica (EPOC) en la provincia de León.Métodos: estudio epidemiológico, transversal, multicéntrico (30 centros de salud de la provincia de León). Incluyó pacientes mayores de 35 años diagnosticados y tratados de EPOC. Variables a estudio: edad, sexo, hábitat, datos antropométricos, tabaquismo, estado nutricional, ejercicio físico, vacunación antigripal, vacunación, antineumocócica (VNP23 y VNC13), fenotipo, gravedad, reagudizaciones y hospitalizaciones. Los resultados se expresan con sus IC al 95,5%.Resultados: se incluyeron 833 pacientes, el 85,8% varones, edad media: 64,69 años (53,66-75,61) y 20,65 años (4,47-36,8) de evolución de la EPOC. El 86,67% (80,30-93,30) tenían antecedentes de tabaquismo (n = 722), de 35,26 años de evolución (17,87-52,64), consumían 28,36 paquetes al año (9,60-46,86), p < 0,001, siendo el 58% fumadores severos. En fumadores activos (n = 288) la intervención más efectiva fue terapia cognitivo-conductual más vareniclina, con abstinencias del 29,86%. En total dejaron de fumar el 51,05% (49,49-52,70) de los pacientes con EPOC, p < 0,001. El 73,67% (71,78-75,65) realizaba ejercicio prescrito, el 88,76% (84,82-90,7) realizaba dieta equilibrada, el 89,7% (87,8-91,8) estaba vacunado frente a la gripe, siendo esta más frecuente en los mayores de 65 años y hospitalizados, p < 0,001. El 9,61% (7,7-11,6) de los no vacunados tuvo reagudizaciones que requirieron ingreso hospitalario, p < 0,001. La tasa de vacunación con VNP23 fue del 52,8% (49,3-56,4) vs.4,97% (3,0-6,61) de VNC13, p < 0,05.Conclusiones: las actividades preventivas en los pacientes con EPOC se realizan de forma óptima en nuestro entorno, superior a la media nacional, aunque se deben lograr mayores tasas de cobertura de vacunación frente al neumococo.

    318 records of male workers, 169 Spanish and 149 Arab were retrospectively studied in 1987 at the "Gabinete de Seguridad e Higiene en el Trabajo" (Council for Safety and Hygiene in the Workplace) in Ceuta in order to prove the hypothesis that 2 different ethnic groups living in the same geographic area have a non-equal distribution of cardiovascular risk factors. The Spanish group showed a higher prevalence in blood hypertension, diabetes, glucose intolerance, obesity and alcohol intake, compared to the Arab group. Smoking and high levels of seric cholesterol were similar in both groups, however, medium levels of seric cholesterol were lower in the Arab group. Family histories of cardiovascular disease were very rare in the latter mentioned group. These observations suggested a major predisposition to ischemic cardiopathy in the Spanish group.

    The school health program in Mexico, directed by the Office of School Hygiene (la Unidad de Higiene Escolar), is in a state of flux. The program will change substantially if an initiative between the national offices of health and education is enacted. The initiative would establish a national commission to be replicated at state, county, and district levels. Commissions would oversee integration of the health services component, social participation, and research into the school health program which currently only focuses on health instruction and a healthy school environment. The initiative would restore and improve a former model that incorporated health services as a part of the school health program. The history of the school health program, which can trace its roots to 1861 and President Benito Juarez, is provided.

  14. [Programa de saúde: a case of book censorship during the Brazilian military dictatorship].

    Some documents produced by the government during the Brazilian military dictatorship have been made available for reference, including censors' reports on television and radio programs, plays, films, songs, advertisements and printed matter. Of the 500 or so reports on books, there is one that vetoes Programa de saúde: projetos e temas de higiene e saúde, an educational work that proposed actions to help students understand their sanitary conditions and acquire knowledge about health and health-promoting habits in this area. The argument for vetoing it was that the information could fall into the hands of poorly trained teachers, who could distort its content, causing serious damage to Brazilian youth. In this report we see evidence of the obscurantist, elitist posture adopted by the Department of Censorship of Public Entertainment, which aimed to restrict the circulation of information and curb debate.

  15. Report on the activities carried out by 'Sonrisas' to promote oral health: the experience of a Canadian dental hygienist in the Dominican Republic.

    Smiles Foundation is a non-profitable organization established in Canada as a sister foundation to the Dominican Fundacion para la Prevencion y la Salud Bucal de los Ninos "Sonrisas" the Dominican Republic, which provides free dental treatment and oral health education to underprivileged children. Its founder Elina Katsman, a Canadian dental hygienist embarked on this project in 1986, and started giving lectures in the small community of Samana along with two local nurses that she trained in the principles of oral hygiene and related primary health care. Today, the foundation runs eight dental clinics and five mobile units that serve remote areas and has benefited 1,723,119 people in total. The aim of this article was to report on the growth that Smiles Foundation has experienced to promote oral health among children and adults in the Dominican Republic including its activities and the results it has achieved up until 2005.

    In Peru, the industry's development has made economic improvements but at the same time, it has had a major impact on the health of the workers; for that reason, it was necessary to generate control mechanisms. So, in 1940 it was created the Departmento de Higiene Industrial, which in 1956 was changed to Instituto de Salud Ocupacional, but it was deactivated in 1994. However, in 2001 it reappeared into the Ministerio de Salud organizational structure with the name of Instituto de Salud Ocupacional "Alberto Hurtado Abadía". Actually, it is the Centro Nacional de Salud Ocupacional y Protección del Ambiente para la Salud (CENSOPAS), organ of the Instituto Nacional de Salud which continues working in synergy with other institutions and sectors, making research to protect the health of exposed persons (workers and community) to contamination and risks associated with economic activities.

    El cáncer de páncreas es la cuarta causa de muerte por cáncer en los Estados Unidos; en el mundo se asocia con 227.000 muertes anuales, aproximadamente. Es producto de múltiples factores, siendo el tabaquismo el principal factor de riesgo.La punción-aspiración con aguja fina guiada por ultrasonido endoscópico es una técnica muy eficaz en el diagnóstico de lesiones neoplásicas del páncreas. El diagnóstico citológico mediante esta técnica debe hacerse según los lineamientos para el sistema pancreático-biliar de la Papanicolaou Society of Cytopathology. Dichos lineamientos incluyen las indicaciones, las técnicas, la terminología y la nomenclatura, así como los estudios auxiliares, el manejo posterior al procedimiento y los criterios citológicos para el diagnóstico.La especificidad de una interpretación positiva o maligna para la punción-aspiración pancreática con aguja fina, es de 90 a 95 % en la mayoría de los estudios.

    The status of Chagas' disease in Ecuador is not clear. In response to reports suggesting the possibility of transfusion-associated transmission of Chagas' disease in the blood bank in Quito, the Ecuadorian Red Cross in collaboration with the Instituto Nacional de Higiene, Zona Norte and the Tropical Disease Institute of Ohio University implemented a pilot Chagas' disease screening of the donated blood in the Quito blood bank. The results of the screening showed a low incidence of seropositivity among the donors (0.01% in 1994, 0.04% in 1995, and 0.02% in 1996) to the Quito blood bank and a higher seropositivity in samples donated to smaller blood banks (0.4% in 1994, 0.28% in 1995, and 0.13% in 1996) located in areas considered endemic, as well as from at least two areas previously considered nonendemic for Chagas' disease. This report highlights the need for a comprehensive evaluation of the prevalence and distribution of Chagas' disease in Ecuador.

  19. A new system to reduce formaldehyde levels improves safety conditions during gross veterinary anatomy learning.

    Dissection is a very useful method of learning veterinary anatomy. However, formaldehyde, which is widely used to preserve cadavers, is an irritant, and it has recently been classified as a carcinogen. In 1997, the Instituto Nacional de Seguridad e Higiene en el Trabajo [National Institute of Workplace Security and Hygiene] found that the levels of formaldehyde in our dissection room were above the threshold limit values. Unfortunately, no optimal substitute for formaldehyde is currently available. Therefore, we designed a new ventilation system that combines slow propulsion of fresh air from above the dissection table and rapid aspiration of polluted air from the perimeter. Formaldehyde measurements performed in 2004, after the introduction of this new system into our dissection laboratory, showed a dramatic reduction (about tenfold, or 0.03 ppm). A suitable propelling/aspirating air system successfully reduces the concentration of formaldehyde in the dissection room, significantly improving safety conditions for students, instructors, and technical staff during gross anatomy learning.

    Tobacco use presents an odd confluence of circumstances: it is a significant and high threat to health, and there is a lack of motivation among health workers to act accordingly. Yet we have effective interventions. It is really hard to identify any other determinant of health presenting this mixture of lethality, prevalence, and lack of care, despite having effective treatments readily available. On the other hand, smoking cessation interventions are considered as the gold standard of preventive interventions, far above other preventive measures commonly used. This has prompted the National Committee for Smoking Prevention to develop a consensus document for the Clinical Care of Smoking Cessation in Spain. The purpose of this technical and scientific document is to agree on a basic proposal of quality of care to tackle smokers to quit. This document would serve as a guideline in the clinical practice in our country. The aims of this agreement are to review the effectiveness of the existing therapies for smoking cessation, to synthesize their available evidence, and to set the basic minimum standards of care in the clinical practice of patients who smoke. The consensus sets the strategies, and the evidences that support them, in order to assist both the smokers who want to quit, and the smokers who do not, setting out the steps to intervene in the most adequate.   1. Mouthpiece ventilation in Duchenne muscular dystrophy: a rescue strategy for noncompliant patients.

    To evaluate mouthpiece ventilation (MPV) in patients with Duchenne muscular dystrophy (DMD) who are noncompliant with noninvasive ventilation (NIV). We evaluated four young patients with DMD who had previously refused to undergo NIV. Each patient was reassessed and encouraged to try MPV. The four patients tolerated MPV well and were compliant with NIV at home. MPV proved to be preferable and more comfortable than NIV with any other type of interface. Two of the patients required overnight NIV and eventually agreed to use a nasal mask during the night. The advantages of MPV over other types of NIV include fewer speech problems, better appearance, and less impact on the patient, eliminating the risk of skin breakdown, gastric distension, conjunctivitis, and claustrophobia. The use of a mouthpiece interface should be always considered in patients with DMD who need to start NIV, in order to promote a positive approach and a rapid acceptance of NIV. Using MPV during the daytime makes patients feel safe and more likely to use NIV at night. In addition, MPV increases treatment compliance for those who refuse to use other types of interfaces. Avaliar a ventilação bucal (VB) em pacientes com distrofia muscular de Duchenne (DMD) não aderentes à ventilação não invasiva (VNI). Foram avaliados quatro pacientes jovens com DMD que anteriormente recusaram-se a se submeter à VNI. Cada paciente foi reavaliado e encorajado a tentar VB. Os quatro pacientes toleraram bem a VB e aderiram ao uso de VNI em casa. O uso de VB provou ser uma alternativa preferível e mais confortável que o uso de VNI com qualquer outro tipo de interface. Dois dos pacientes necessitaram de VNI noturna e eventualmente aceitaram utilizar uma máscara nasal durante a noite.     The production of Pertussis Vaccine was reevaluated at the Instituto Nacional de Higiene "Rafael Rangel" in order to optimise it in terms of vaccine yield, potency, specific toxicity and efficiency (cost per doses). Four different processes, using two culture media (Cohen-Wheeler and Fermentación Glutamato Prolina-1) and two types of bioreactors (25 L Fermentador Caracas and a 450 L industrial fermentor) were compared. Runs were started from freeze-dried strains (134 or 509) and continued until the obtention of the maximal yield. It was found that the combination Fermentación Glutamato Prolina-1/industrial fermentor, shortened the process to 40 hours while consistently yielding a vaccine of higher potency (7.91 +/- 2.56 IU/human dose) and lower specific toxicity in a mice bioassay. In addition, the physical aspect of the preparation was rather homogeneous and free of dark aggregates. Most importantly, the biomass yield more than doubled those of the Fermentador Caracas using the two different media and that in the industrial fermentor with the Cohen-Wheeler medium. Therefore, the cost per doses was substantially decreased.

    This study was conducted to investigate the association between prolonged breastfeeding and early childhood caries(ECC) with adjustment for important confounders, using hieraschical approach. This retrospective cohort study involved 260 low-income children (18-42 months). The number of decayed teeth was used as a measure of caries. Following a theoretical framework, the hierarchical model was built in a forward fashion, by adding the following levels in succession: level 1: age; level 2: social variables; level 3: health variables; level 4: behavioral variables; level 5: oral hygiene-related variables; level 6: oral hygiene quality measured by visible plaque; and level 7: contamination by mutans streptococci. Sequential forward multiple Poisson regression analysis was employed. Breast-feeding was not a risk factor for ECC after adjustment for some confounders (incidence density ratio, 1.15; 95% confidence interval, 0.84-1.59, P = 0.363). Prolonged breast-feeding was not a risk factor for ECC while age, high sucrose comption between main meals and the quality of oral higiene were associated with disease in children. © 2012 John Wiley & Sons A/S.

    Introducción La gastroquisis es una malformación de etiología desconocida en la que se han implicado factores de riesgo (FR) genéticos y medioambientales. El objetivo de este trabajo es desarrollar la historia clínica medioambiental pediátrica (HCMAP) de la gastroquisis en 2 pacientes. Pacientes y métodos Revisión bibliográfica en Pubmed y en el Developmental and Reproductive Toxicology Database. Búsqueda selectiva de sustancias con capacidad teratogénica en el Hazardous Substances Data Bank. Palabras clave utilizadas fueron: «gastroschisis» y «gastroschisis and risk factor». Resultados En ambos casos están presentes los siguientes FR descritos en la literatura: corta cohabitación, embarazos no planificados de madres relativamente jóvenes, cambio de pareja reciente, ingesta de alcohol, déficits nutricionales importantes, tabaquismo activo/pasivo. Un caso estuvo expuesto a cocaína, humo de cannabis y se realizó una ortopantografía durante el embarazo. Conclusiones 1) Es necesario obtener la HCMAP en todo paciente con gastroquisis; 2) una cuidadosa HCMAP requiere una adecuada revisión de los FR relacionados e instrucción básica para caracterizar y cuantificar las exposiciones medioambientales, y 3) siguiendo estos pasos, complementaremos nuestras labores asistenciales y preventivas.   6. Factores predictores de inicio y cesación de tabaquismo en una cohorte de mujeres chilenas con 5,5 años de seguimiento

    Background Chilean women have one of the highest smoking prevalence in the world. Aim To estimate the main factors associated with smoking initiation and quitting among a cohort of adult women living in a low socioeconomic status area of Santiago, Chile. Material and methods A random population-based sample of 1,100 women, 18 years and older, were selected from a community located in the South East area of Santiago. Sociodemographic, as well as smoking, beliefs, behaviors, stages of change and nicotine addiction level were recorded during a personal interview. After an average follow-up period of 5.5 years, women were re-evaluated. Results Seventy-three percent of women completed the study. At baseline, 39% of women were smokers. At the end of the study, there was an absolute smoking rate reduction of 7.1% (p<0.001). The main variables associated with smoking initiation were younger age (Odds ratio (OR): 1.08, 95% confidence intervals (CI): 1.05–1.12), higher education level (OR: 1.2, 95% CI: 1.07–1.35), and having fewer children (OR: 1.3 95% CI: 1.01–1.66). Factors related with quitting were younger age of onset (OR: 1.06 95% CI: 1.02–1.1), higher level of nicotine dependence (OR: 4.22, 95% CI: 1.74–10.27), and higher perception of smoking addiction (OR: 4.34, 95% CI: 2–9.09). Stage of change was associated with smoking cessation but its effect was diluted after adjusting for the level of nicotine addiction. Conclusions Sociodemographic and family factors were the main variables related with initiation, whereas age of onset, belief of addiction, and nicotine dependence were the main factors related with cessation. Women with a high motivation for quitting should be evaluated for nicotine addiction level to define the best strategy for intervention (Rev Méd Chile 2009; 137: 1001–9). PMID:19915762

  7. Comparative study of knowledge about oral cancer among undergraduate dental students.

    To evaluate and compare the knowledge of dental undergraduate students about oral cancer. The students were divided into two groups according to semester attended in the undergraduate course: Group A, the first semester; and Group B, seventh semester. They were asked to answer a questionnaire about epidemiology, risk factors, clinical aspects, therapeutic solutions and oral self-examination. For statistical analysis, the Fisher's exact test, the Cochran's G test and Kendall's concordance test were used, with significance level set at 0.05. Regarding the prevalent sex, only 8.0% of Group A and 56.0% of Group B judged males as the frequent affected by the disease (p=0.0006). In terms of age, 84.0% of the Group B and 44.0% of the Group A estimated that most cases were diagnosed over 40 years (p=0.0072). Smoking was identified as the major risk factor for 64.0% and 91.6% of Groups A and B, respectively (p=0.0110). On issues related to sex, ethnicity, age, risk factors, self-examination, treatment, professional responsible for treatment and profile of an individual with the disease, the seventh-semester showed significantly higher correct answer percentages than first-semester undergraduates. There was significant correlation between the right and wrong answers given by first and seventh semester students, making necessary a specific approach directed to their lack of knowledge.   8. A new ELISA for determination of potency in snake antivenoms.

    A competitive ELISA for potency determination of bothropic equine antivenom was developed and compared to the conventional in vivo ED(50) assay, with the aim of partially substituting the in vivo assay in the monitoring of antivenom immunoglobulin levels. On this purpose, blood samples were taken at different times during and after the immunization protocol of the lot of horses used for production of snake antivenom at the Instituto de Higiene, Uruguay. Both the competitive ELISA and the ED(50) assay were performed on those samples. In addition, a group of five commercial pepsin-digested antivenoms were tested by both methods. A significant (P<0.001) correlation (Pearson's r=0.957) was found between the ELISA titres and the corresponding ED(50) values, indicating that the in vitro test can estimate the neutralizing antibody capacity of the sera as well as the in vivo assay. By means of this new ELISA, it was found that the immunized animals maintained good venom antibody titres, in the order of 20-50% of the maximum achieved, even 10 month after the end of the immunization schedule. The main advantage of our ELISA design is its ability to correctly estimate the neutralization capacity of crude hyperimmune plasma and antivenom sera independently of their antibody composition in terms of whole IgG or F(ab')(2) fragment.

    Introducción: El tabaquismo es la primera causa evitable de muerte en el mundo. El cigarrillo electrónico (CE), es un dispositivo que simula a los cigarrillos convencionales y permite inhalar nicotina y otras sustancias de forma vaporizada, sin combustión de tabaco. Su conocimiento por parte de la población general, así como su comercialización y consumo viene en constante aumento. Es utilizado en todo el mundo con el objetivo de disminuir el consumo de tabaco, lograr su abandono o poder utilizarlo en lugares públicos donde el consumo de cigarrillos convencionales está prohibido. Efectos nocivos sobre la salud: Su utilización se ha asociado a neumonía lipoidea e irritación de la vía aérea superior y toxicidad por nicotina. Presenta sustancias cancerígenas como nitrosaminas, formaldeido y metales como el niquel, cromo y plomo. Aumenta la resistencia de la vía aérea, efecto que es similar al que se produce después de la inhalación del humo del tabaco. Eficacia para dejar de fumar: No hay hasta el momento trabajos que demuestren con poder estadístico y buena metodología que este producto sea eficaz para dejar de fumar.  Conclusiones: De acuerdo a la evidencia disponible, no podemos descartar que el uso del cigarrillo electrónico no tenga riesgos para la salud. La seguridad y eficacia de los cigarrillos electrónicos como ayuda para el abandono del hábito tabáquico no han sido demostradas.

    Resumen Objetivos El objetivo de este estudio fue estudiar la asociación de la adherencia a la dieta mediterránea materna al inicio del embarazo y el riesgo de gastrosquisis en la descendencia. Métodos Estudio de casos-control. 11 casos incidentes de gastrosquisis en la Región de Murcia de 2007 a 2012 y 34 controles concurrentes. Cuestionario validado de Frecuencia Alimentaria (CFA) sobre la dieta periconcepcional de 98 ítems realizado ‘cara a cara’ en el momento del diagnóstico. Factores confundidores: tabaquismo, expositión a cannabis/marihuana, edad materna y paterna, índice de masa corporal, ingresos económicos y nivel de estudios. Estudio descriptivo y regresión logística multivariable. Resultados Las madres de niños con gastrosquisis son más jóvenes (20,8 años; IC 95% 17,3–24,2) y su dieta tiene un menor aporte calórico, de grasas saturadas y monoinsaturadas y de proteínas que los controles. Odds Ratio (OR) en el modelo multivariable controlado por los factores confundidores: edad materna (años) 0,70 (IC95% 0,51–0,96); ácidos grasos monoinsaturados (oleico, g) 0,79 (IC95% 0,65–0,97) y consumo de vegetales (raciones/semana) 0,70 (IC95% 0,48–1,00). Conclusiones Una dieta materna rica en ácido oleico y productos vegetales podría contribuir a prevenir el riesgo de oclusión vascular de las arterias onfalomesentéricas, disminuyendo el riesgo de gastrosquisis.   11. [Association between preventive care provided in public dental services and caries prevalence].

    To assess the association between preventive care provided in public dental services and young people's oral health. Oral health data on 4,033 young people aged 15 to 19 years living in 85 municipalities of the state of Rio Grande do Sul, Southern Brazil, were obtained from the national oral health survey "Saúde Bucal Brasil 2003" for the period 2002-2003. The following variables were studied: age, gender, income, education, time elapsed since last dental visit, reason for dental visit, and water fluoridation. Data on dental care services were obtained from the national database of public health services. Statistical analysis was performed using multilevel logistic regression. Youngsters from the 21 municipalities with the lowest preventive care (scaling + fluoride + sealants) rates per 100 inhabitants were 2.27 (95% CI: 1.45;3.56) more likely to have non-filled dental cavities than those from the 21 municipalities with the highest care rates. After adjustment for a number of individual and contextual factors this likelihood decreased to 1.76 (95% CI: 1.13;2.72). The variance attributable to variables at municipal level was 14.1% for the empty model and decreased to 10.5% for the fully adjusted model. Rio Grande do Sul public dental services may have contributed for the reduction in the number of non-filled cavities in young people. However, it was not possible to detect the impact of this service on total dental caries experience.

  12. Accuracy of different impression materials in parallel and nonparallel implants

    Background: A precise impression is mandatory to obtain passive fit in implant-supported prostheses. The aim of this study was to compare the accuracy of three impression materials in both parallel and nonparallel implant positions. Materials and Methods: In this experimental study, two partial dentate maxillary acrylic models with four implant analogues in canines and lateral incisors areas were used. One model was simulating the parallel condition and the other nonparallel one, in which implants were tilted 30° bucally and 20° in either mesial or distal directions. Thirty stone casts were made from each model using polyether (Impregum), additional silicone (Monopren) and vinyl siloxanether (Identium), with open tray technique. The distortion values in three-dimensions (X, Y and Z-axis) were measured by coordinate measuring machine. Two-way analysis of variance (ANOVA), one-way ANOVA and Tukey tests were used for data analysis (α = 0.05). Results: Under parallel condition, all the materials showed comparable, accurate casts (P = 0.74). In the presence of angulated implants, while Monopren showed more accurate results compared to Impregum (P = 0.01), Identium yielded almost similar results to those produced by Impregum (P = 0.27) and Monopren (P = 0.26). Conclusion: Within the limitations of this study, in parallel conditions, the type of impression material cannot affect the accuracy of the implant impressions; however, in nonparallel conditions, polyvinyl siloxane is shown to be a better choice, followed by vinyl siloxanether and polyether respectively.   13. Effect of whitening dentifrices on the surface roughness of commercial composites.

    Our study aimed to test the null hypothesis that whitening and non-whitening dentifrices affect similarly the surface roughness of commercial microhybrid composites, independent of the brushing time. One hundred and ninety-two disc-shaped specimens of Filtek Z250 (3 M/ESPE, St. Paul, MN, USA) and Rok (SDI, Australia) were built up and randomly assigned to 24 groups, based on the dentifrices used (two whitening dentifrices: Colgate Max White-Colgate-Palmolive, São Bernardo do Campo, São Paulo, Brazil and Close Up Extra Whitening-Unilever, Brasil Higiene Pessoal e Limpeza Ltda, Ipojuca, Pernambuco, Brazil; and one non-whitening dentifrice: Colgate Total 12 Clean Mint-Colgate-Palmolive), and on the simulated brushing times (24 hours, 6, 12 and 24 months). The specimens were submitted to the toothbrushing regimens after which the surface roughness (Ra) was measured. Data was submitted to analysis of variance and Tukey test (α=0.05). The composite's surface roughness was significantly affected by the composites (p=0.0007), the dentifrices (p=0.0001), and the simulated brushing time (p=0.0001). Higher roughness was observed when the whitening dentifrices were used and when the brushing time increased. Filtek Z250 was more affected than Rok, especially after 24 months of simulated brushing. Whitening dentifrices produced higher surface roughness in the composites tested. The degree of surface compromising increased with brushing time and depends on the composite's microstructure and composition. © 2011 Wiley Periodicals, Inc.

  14. Molecular characterization of hepatitis E virus in patients with acute hepatitis in Venezuela.

    Hepatitis E virus (HEV) causes a common infection in developing countries. HEV infection occurs as outbreaks, as sporadic clinical cases and as large epidemics in endemic areas. The objective of this study was to determine the presence of HEV infection in patients with clinical suspicion of hepatitis A virus (HAV) infection, referred to the Instituto Nacional de Higiene "Rafael Rangel" in Venezuela. Seventy-four sera were tested for anti-HAV and anti-HEV IgM antibodies. HEV-RNA was amplified from anti-HEV IgM positive sera using nested reverse transcription polymerase chain reaction for ORF1 (RNA dependent RNA polymerase region) and the amplicons sequenced for phylogenetic analysis. The frequency of anti-HEV IgM was 22/74 (30%) in the samples tested. Dual infection with HAV and HEV was found in 31% (12/39) of anti-HAV IgM positive patients. Viremia was detected in 3/22 (14%) of sera positive for anti-HEV IgM. Two HEV strains were classified as genotype 1 and one as genotype 3, which were closely related to Yam 67 (north of India) and US1 isolates from the USA, respectively. These findings suggest that HEV is an important cause of acute viral hepatitis in Venezuela as a single infection or co-infection with HAV, with high morbidity in children and young adults suggesting that this infection is endemic in Venezuela.     Professor Rafael Molla Rodrigo is considered by some the author who contributed with his scientific work and solid technique to consolidate Urology as a speciality in Valencia. Such importance is also perceived in a national level, as demonstrated by the fact that he belonged to the first board of the Spanish Association of Urology and contributed to its foundation in the two-year period 1910--1911, from his position of head of the department of Surgical Therapy and Operations at the Central University. Genitourinary tuberculosis is one of the most outstanding chapters of his extensive works. We reviewed all his works, selecting all original articles about genitourinary tuberculosis. Original articles have been obtained from the journals Revista de Higiene y Tuberculosis, La Medicina Valenciana, Revista Valenciana de Ciencias Médicas y Policínica, and from his two most important books, Clinical Lessons In Urology and Clinical Lessons in Urology and Genitourinary Surgery. Throughout his work he analyzes features of relevance about this disease, such as diagnosis, about which he emphasizes the clinical and bacteriological difficulties, and treatment, which was only feasible by precocious nephrectomy. He dedicates a great part of his work to the study of this pathology and defends a precocious diagnosis in order to perform the only treatment that had demonstrated efficacy. He also exposes the symptoms and variety of clinical presentations, as well as diagnosis through separate kidney catheterization with the aim to determine unilateral involvement.

    The aim of the study was to quantify the scientific productivity of researchers, organizations, and regions in Spain that publish articles on implantology in dental journals indexed in Journal Citation Reports. A search was conducted among the core collection of Thomson Reuters' Web of Science database, on the basis of its broad thematic and geographic coverage of health sciences. The search identified original articles - the main vehicle for the dissemination of research results. The search was conducted in July 2016, applying the truncated search term 'implant*' to locate original articles on implantology and its derivative forms. The search was conducted within the topic field (title, keywords and abstract) and two inclusion criteria were applied: documents denominated as articles were included; and articles categorized as Web of Science Medicine Dentistry and Oral Surgery. Finally only articles for which one of the participating organizations was located in Spain were selected. The final search identified a total of 774 records. The period 1988 to 2015 saw an exponential growth in scientific production, especially during the last 10 years. Clinical Oral Implants Research and Medicina Oral Patologia Oral y Cirugia Bucal (Oral Medicine, Oral Pathology, and Oral Surgery) were the most productive journals. Collaborative networks among authors and among institutions increased and this increase was related to the improving quality of the publications. Bibliometric analysis revealed a significant growth in the quantity and quality of Spanish implantology literature. Most key bibliometric indicators demonstrated upward trends.     This work presents the sampling procedure used to select the sample got for a "Human Reproduction Study in the District of São Paulo" (Brazil), done by the Department of Applied Statistics of "Faculdade de Higiene e Saúde Pública da Universidade de São Paulo". The procedure tried to solve the situation which resulted from the limitation in cost, time and lack of a frame that could be used in order to get a probability sample in the fixed term of time and with the fixed cost. It consisted in a two stage sampling with dwelling-units as primary units and women as secondary units. At the first stage, it was used stratified sampling in which sub-districts were taken as strata. In order to select primary units, there was a selection of points ("starting points") on the maps of subdistricts by a procedure that was similar to that one called "square grid" but differed from this in several aspects. There were fixed rules to establish a correspondence between each selected "starting point" and a set of three dwelling units where at least one woman of the target population lived. In the selected dwelling units where more than one woman of target population lived, there was a sub-sampling in order to select one of them. In this selection each woman living in the dwelling unit had equal probability of selection. Several "no-answer" cases and correspondent instructions to be followed by the interviewers are presented too.

  18. Learning together: a Canada-Cuba research collaboration to improve the sustainable management of environmental health risks.

    To build a national Cuban capacity for training environmental health professionals directly linked to the needs of policy-makers and communities. The University of Manitoba and University of British Columbia collaborated with an established training centre in Cuba (the Instituto Nacional de Higiene y Epidemiologia--INHEM) and new centres in the Central (Santa Clara) and Eastern (Santiago) regions of the country. Cuba. In the mid-1990s, a comprehensive curriculum (masters and diploma programs) was collaboratively developed, applying interactive teaching methods, and was delivered through a series of workshops and other interactions in Cuba, and short-term visits to Canada by Cuban PhD students. The collaboration was successful in fulfilling capacity-building targets (over 50 Masters graduates, 467 Diploma graduates, over 30 trained core faculty in all regional centres as well as new curriculum and new accredited regional programs). Alongside this, a number of collaborative community-based research projects were undertaken in all three regions (drinking water in Santiago; housing and urban renewal, and dengue control in Havana; and tourism-related effects, and effective intersectoral management of population health determinants in Santa Clara). The collaboration led to adopting new strategies for challenges such as a dengue epidemic in 2002, and new research on the effectiveness of intersectoral management of risks of particular interest to both Cuban and Canadian policy-makers. It triggered an ambitious collaboration between the Canadian-Cuban team and colleagues in Ecuador in order to build a similar national network there, built on South-South and North-South links.

    Introducción Estudios internacionales demuestran que existe un patrón diferenciado de salud y una disparidad en la atención sanitaria entre personas con discapacidad intelectual (DI) y población general. Objetivo Obtener datos sobre el estado de salud de las personas con DI y compararlos con datos de población general. Pacientes y métodos Se utilizó el conjunto de indicadores de salud P15 en una muestra de 111 sujetos con DI. Los datos de salud encontrados se compararon según el tipo de residencia de los sujetos y se utilizó la Encuesta Nacional de Salud 2006 para comparar estos datos con los de la población general. Resultados La muestra con DI presentó 25 veces más casos de epilepsia y el doble de obesidad. Un 20% presentó dolor bucal, y existió una alta presencia de problemas sensoriales, de movilidad y psicosis. Sin embargo, encontramos una baja presencia de patologías como la diabetes, la hipertensión, la osteoartritis y la osteoporosis. También presentaron una menor participación en programas de prevención y promoción de la salud, un mayor número de ingresos hospitalarios y un uso menor de los servicios de urgencia. Conclusiones El patrón de salud de las personas con DI difiere del de la población general, y éstas realizan un uso distinto de los servicios sanitarios. Es importante el desarrollo de programas de promoción de salud y de formación profesional específicamente diseñados para la atención de personas con DI, así como la implementación de encuestas de salud que incluyan datos sobre esta población.     To determine the Melilla Hospital Nursing Department's attitude towards Moroccan patients. Descriptive ex post facto study. A questionnaire has been handed over to staff, on the Immigration Attitude Scale for Nursing. In general, nurses exhibit negative attitudes towards Moroccan patients, such as: the increase in crime is caused by the arrival of immigrants, those who commit offenses must be expelled from Spain, they take advantage of the Spanish health system and too many resources are devoted to immigration. The worst-rated immigrants are the Moroccans, considering that they do not pay much attention to their personal hygiene and do not adapt to their host countries. It is necessary to work with the nursing staff to change these attitudes. Future degree students must be trained in cultural skills and the care of immigrants will improve with a greater commitment towards cultural differences. Se ha pasado un cuestionario de Escala de Actitud ante la Inmigración para Enfermería. En general las enfermeras presentan actitudes negativas ante los pacientes marroquíes, como: el aumento de la delincuencia es provocado por la llegada de inmigrantes, los que delinquen deben ser expulsados de España, se aprovechan del sistema sanitario y se dedican demasiados recursos para la inmigración. Los inmigrantes peores valorados son los marroquíes, considerando que son pocos cuidadosos con su higiene personal y no se adaptan a los países de acogida. Es necesario trabajar con el personal de enfermería para que cambien estas actitudes. Las futuras promociones de Grado deben estar formadas en competencias culturales y mejorarán los cuidados a los inmigrantes como un mayor compromiso con la diferencia cultural.

    To compare the number of appropriate behaviors for tooth brushing before and after a playful learning intervention with preschool children. A quasi-experimental, quantitative, before and after study design was conducted in an early childhood educational institution, with children between three and five years of age. The intervention consisted of three meetings with educational activities about tooth brushing, whose outcome was evaluated by means of observation of ten behaviors suitable for tooth brushing. Forty-four children participated in the study. The mean of adequate behaviors was 4.4 before the intervention, and 8.5 after the intervention. A significant increase in the adoption of appropriate behaviors for tooth brushing (p <0.01) was identified. Nurses can enhance oral health promotion actions with preschoolers in preschool institution using playful learning interventions. Comparar o número de comportamentos adequados para a escovação de dentes antes e após uma intervenção educativa lúdica com pré-escolares. Estudo quase-experimental, do tipo antes-depois, com abordagem quantitativa e conduzido em instituição de educação infantil com crianças entre três e cinco anos de idade. A intervenção consistiu em três encontros com atividades educativas lúdicas sobre escovação de dentes, cujo efeito foi avaliado por meio da observação de dez comportamentos adequados para a escovação dos dentes. Participaram do estudo 44 crianças. A média de comportamentos adequados foi de 4,4 antes da intervenção e 8,5 após a mesma. Houve um aumento significativo na adoção de comportamentos adequados para a escovação de dentes (p < 0,01). Por meio de intervenções educativas lúdicas, recomenda-se que os enfermeiros potencializem as ações de promoção da saúde bucal com pré-escolares em instituições de educação infantil.

  4. Effectiveness of two-sided UV-C treatments in inhibiting natural microflora and extending the shelf-life of minimally processed 'Red Oak Leaf' lettuce.

    The use of UV-C radiation treatments to inhibit the microbial growth and extend the shelf-life of minimally processed 'Red Oak Leaf' lettuce was investigated. Initially, UV-C resistance of 20 bacterial strains from different genera often associated with fresh produce (Enterobacter, Erwinia, Escherichia, Leuconostoc, Pantoea, Pseudomonas, Rahnela, Salmonella, Serratia and Yersinia) were tested in vitro. Most of the bacterial strains were inhibited with the minimum dose (30 J m(-2)). Erwinia carotovora, Leuconostoc carnosum, Salmonella typhimurium, and Yersinia aldovae were the most resistant strains requiring a UV-C dose of 85 J m(-2) to completely inhibit growth. An in vivo study consisted of treating minimally processed 'Red Oak Leaf' lettuce (Lactuca sativa) with UV-C at three radiation doses (1.18, 2.37 and 7.11 kJ m(-2)) on each side of the leaves and storing the product under passive MAP conditions at 5 degrees C for up to 10 days. The gas composition inside packages varied significantly among the treatments, with CO2 concentrations positively and O2 concentrations negatively correlating with the radiation dose. All the radiation doses were effective in reducing the natural microflora of the product, although the highest doses showed the greatest microbial inhibitions. Taking into account the microbial limit set by Spanish legislation [Boletín Oficial del Estado (BOE), 2001. However, the 7.11 kJ m(-2) dose induced tissue softening and browning after 7 days of storage at 5 degrees C. Therefore, the use of two sided UV-C radiation, at the proper dose, is effective in reducing the natural microflora and extending the shelf-life of minimally processed 'Red Oak Leaf' lettuce.

    este artículo discute brevemente la prevalencia de caries, la naturaleza multifactorial de su etiología, el riesgo de caries y el papel y eficacia del fluoruro. Resalta también la investigación sobre el metabolismo bacteriano, que ha aportado conocimientos sobre la defensa natural oral contra la caries y la base para el desarrollo de una nueva tecnología para la prevención diaria y el tratamiento de la caries. Por último, se resume la evidencia que respalda que la tecnología complementa y mejora la eficacia anti-caries de la crema dental con fluoruro. los datos globales muestran que a pesar de la exitosa introducción del fluoruro, la caries dental es una enfermedad prevalente. La experiencia de caries depende del balance entre el consumo de azúcares, la higiene oral y el uso del fluoruro. Hay tres conceptos científicos que son fundamentales en las nuevas mediciones para detectar, tratar y monitorear la caries: (1) la caries dental es un proceso dinámico, (2) la caries dental es un proceso continuo de etapas que van desde reversible (pre-clínica) hasta irreversible (lesiones clínicamente detectables), y (3) el proceso de la caries es un balance de factores patológicos y protectores que pueden modularse para el manejo de la caries. El fluoruro funciona como factor protector al detener y revertir el proceso de la caries, pero el fluoruro no previene los factores patológicos que inician el proceso. Se ha identificado una tecnología novedosa, basada en arginina y un compuesto insoluble de calcio, que está dirigida a la placa dental para prevenir la iniciación del proceso de caries al reducir los factores patológicos. Como los mecanismos de acción de la arginina y el fluoruro son altamente complementarios, se ha desarrollado un nuevo dentífrico que combina la arginina y el fluoruro, y se ha probado clínicamente que brinda una prevención superior contra la caries.     In Peru, the incidence of gastric cancer is reported to be around 15.8 per 100,000 inhabitants and it is the second most common oncological disease in men and the third one in women. Additionally, a high mortality index was reported, especially among poor people. To address this issue, in 2008, Peru initiated several insurance treatment plans of oncological diseases with promising results. In Mexico, there is a high predominance of gastric cancer in male gender compared to female gender, even reaching a 2/1 ratio, and the detection rate of early gastric cancer is low (10% to 20%) which results in a mainly palliative treatment with an overall survival rate in 5 years about 10% to 15% only. In Peru, the average age at diagnosis is around 62.96±14.75 years old and the most frequent symptoms includes abdominal pain, indigestion, loss of appetite, weight loss and gastrointestinal bleeding, while in Mexico, some studies reported an average age at diagnosis around 60.3±4.1 years old (range, 23-78 years old) and the most frequent symptoms were postprandial fullness (74.4%), abdominal pain (37.2%), weight loss (18.6%), and melena (4.6%). The anemia rate was 65.1% with a mean Hb level of 6.14 g/dL. In Peru, the most common gastric cancer type is the intestinal-type adenocarcinoma (around 34%), followed by the diffuse-type adenocarcinoma (18.7%), whilst among Mexicans, the diffuse-type was reported in 55.2% of cases, the intestinal-type was reported in 28.2% and the undifferentiated-type corresponded to 6%. In both, Peru and Mexico, 90% of the associated factors includes tabaquismo, diets rich in salt, smoked foods, and a sedentary lifestyle. Family inheritance and advanced age and pharmacological-resistant Helicobacter pylori infection are also important. Poverty has been heavily associated with a higher incidence of gastric cancer.     To know the ways and means of comfort perceived by the older adults hospitalized in a medical service. Ethnographic study with a qualitative approach. We conducted semi-structured interviews with 22 older adults and participant observation of care situations. The ways and means of providing comfort are centered on strategies for promoting care mobilized by nurses and recognized by patients(clarifying/informing, positive interaction/communication, music therapy, touch, smile, unconditional presence, empathy/proximity relationship, integrating the older adult or the family as partner in the care, relief of discomfort through massage/mobilization/therapy) and on particular moments of comfort (the first contact, the moment of personal hygiene, and the visit of the family), which constitute the foundation of care/comfort. Geriatric care is built on the relationship that is established and complete with meaning, and is based on the meeting/interaction between the actors under the influence of the context in which they are inserted. The different ways and means of providing comfort aim to facilitate/increase care, relieve discomfort and/or invest in potential comfort.   10. MASTICATORY FUNCTION OF OBESE CANDIDATES TO BARIATRIC SURGERY FROM DISTINCT SOCIOECONOMIC CLASSES.

    Obesity and metabolic syndrome can be labeled as worldwide outbreak; thus, both have led to serious public health problem. Oral health can be worsened by both, obesity and metabolic syndrome. Tooth loss harms masticatory function, essential status to whom will be submitted to bariatric surgery. Assess masticatory function of obese candidates to bariatric surgery, who belong to distinct socioeconomic class range, in order to recognize hazard factors and the bias of socioeconomic factor in this context. Observational cross-section study, with samples comprised by two groups of patients, with distinct socioeconomic class range, one of them belonging to public health system (SUSG) and the other to private clinic (CPG), candidates to bariatric surgery. Were assessed anthropometric data, comorbidities and medicines usage, blood tests, habits and the number of dental functional units. The groups SUSG and CPG were homogeneous taking into account gender (p=0,890) and age range (p=0,170). The number of dental functional units was higher in the private group (p<0.001). The impaired masticatory function was rather present among public group (p<0.001) and female gender (p<0,001). Regarded as blood tests, fasting glucose was higher in female in SUSG (p<0,001). The following hazard factors have corroborated to have patients rated as impaired masticatory function: belong to public service (OR: 8.420, p=0.003), higher age (OR: 1.186, p<0.001), female gender (OR: 0.153, p=0.029), diabetes mellitus (OR: 2.545, p=0.045) and smokers (OR: 2.951, p=0.043). The general health and masticatory function of female SUSG were worse, highlighting the socioeconomic condition as hazard factor.
