Are the Affordable Care Act Restrictions Warranted? A Contemporary Statewide Analysis of Physician-Owned Hospitals.

The Affordable Care Act placed a moratorium on physician-owned hospital (POH) expansion. Concern exists that POHs increase costs and target healthier patients. However, limited historical data support these claims and are not weighed against contemporary measures of quality and patient satisfaction. The purpose of this study was to investigate the quality, costs, and efficiency across hospital types. One hundred forty-five hospitals in a single state were analyzed: 8 POHs; 16 proprietary hospitals (PHs); and 121 general, full-service acute care hospitals (ACHs). Multiyear data from the Centers for Medicare and Medicaid Services Medicare Cost Report and the statewide Health Care Cost Containment Council were analyzed. ACHs had a higher percentage of Medicare patients as a share of net patient revenue, with similar Medicare volume. POHs garnered significantly higher patient satisfaction: mean Hospital Consumer Assessment of Healthcare Providers and Systems summary rating was 4.86 (vs PHs: 2.88, ACHs: 3.10; P = .002). POHs had higher average total episode spending ($22,799 vs PHs: $18,284, ACHs: $18,856), with only $1435 of total spending on post-acute care (vs PHs: $3867, ACHs: $3378). Medicare spending per beneficiary and Medicare spending per beneficiary performance rates were similar across all hospital types, as were complication and readmission rates related to hip or knee surgery. POHs had better patient satisfaction, with higher total costs compared to PHs and ACHs. A focus on efficiency, patient satisfaction, and ratio of inpatient-to-post-acute care spending should be weighted carefully in policy decisions that might impact access to quality health care. An overview of the ACTS Experiments Program is presented. ACTS is being developed and will flight test the advanced technologies associated with: a Ka-band multibeam antenna, onboard signal processing and switching as well as laser communications. A nominal 3 yr experiments program is planned. Through the experiments program, the capabilities of the ACTS system will be made available to U.S. industry, university and government experimenters to test, prove the feasibility and evaluate the key ACTS system technologies. Communication modes of operation using the baseband processor and microwave switch matrix are presented, along with the antenna coverage pattern. Potential experiment categories are also presented and briefly discussed. An overall schedule of activities associated with the experiments program is outlined. Results of the ACTS Experiments Program will provide information vital to successful industry implementation of ACTS technology in a future operational system.

IntAct (freely available at http://www.ebi.ac.uk/intact) is an open-source, open data molecular interaction database populated by data either curated from the literature or from direct data depositions. IntAct has developed a sophisticated web-based curation tool, capable of supporting both IMEx- and MIMIx-level curation. This tool is now utilized by multiple additional curation teams, all of whom annotate data directly into the IntAct database. Members of the IntAct team supply appropriate levels of training, perform quality control on entries and take responsibility for long-term data maintenance. Recently, the MINT and IntAct databases decided to merge their separate efforts to make optimal use of limited developer resources and maximize the curation output. All data manually curated by the MINT curators have been moved into the IntAct database at EMBL-EBI and are merged with the existing IntAct dataset. Both IntAct and MINT are active contributors to the IMEx consortium (http://www.imexconsortium.org). PMID:24234451

Over the last decade, the demand for reliable mobile satellite communications (satcom) for voice, data, and video applications has increased dramatically. As consumer demand grows, the current spectrum allocation at L-band could become saturated. For this reason, NASA and the Jet Propulsion Laboratory are developing the Advanced Communications Technology Satellites (ACTS) mobile terminal (AMT) and are evaluating the feasibility of K/Ka-band (20/30 GHz) mobile satcom to meet these growing needs. U.S. industry and government, acting as co-partners, will evaluate K/Ka-band mobile satcom and develop new technologies by conducting a series of applications-oriented experiments. The ACTS and the AMT testbed will be used to conduct these mobile satcom experiments. The goals of the ACTS Mobile Experiments Program and the individual experiment configurations and objectives are further presented.

This paper examines the recent prominent view in medical ethics that withdrawing life-sustaining treatment (LST) is an act of killing. I trace this view to the rejection of the traditional claim that withdrawing LST is an omission rather than an act. Although that traditional claim is not as problematic as this recent prominent view suggests, my main claim is that even if we accepted that withdrawing LST should be classified as an act rather than as an omission, it could still be classified as letting die rather than killing. Even though omissions are contrasted with acts, letting die need not be, for one can let die by means of acts. The remainder of the paper is devoted to establishing this claim and addresses certain objections to it. © 2013 John Wiley & Sons Ltd.

The NASA Advanced Communications Technology Satellite (ACTS) provides the technologies for very high gain hopping spot beam antennas, on-board baseband routing and processing, and wideband (1 GHz) Ka-band transponders. A number of studies have recently been completed using the experience gained in developing the actual ACTS system hardware to quantify how well the ACTS technology can be used in future operational systems. This paper provides a summary of these study results including the spacecraft (S/C) weight per unit circuit for providing services by ACTS technologies as compared to present-day satellites. The uses of the ACTS technology discussed are for providing T1 VSAT mesh networks, aeronautical mobile communications, supervisory control and data acquisition (SCADA) services, and high data rate networks for supercomputer and other applications.

An experimental evaluation was conducted with NASA-designed self-acting face and circumferential seals for use in the main shaft positions of advanced gas turbine engines. The seals featured Rayleigh step pads (self-acting geometry) for lift augmentation. The tested seals incorporated design improvements over previous self-acting configurations. Self-acting face seals were tested to speeds of 214 m/s (700 ft/sec, 63700 rpm), air pressures of 216.8 N/sq cm abs (314.7 psia), and air temperatures of 688K (778 F). Self-acting circumferential seals were tested to speeds of 183 m/s (600 ft/sec, 47700 rpm), air pressures of 61.8 N/sq cm abs (89.7 psia), and air temperatures of 711 K (820 F). Self-acting face-seals are capable of operating at conditions exceeding conventional seal capability. The limit on speed capability was found to be the flatness of the seal-seat. The self-acting circumferential seal design tested requires further development for use in advanced engines.

As a result of this legislation, more Americans are likely to qualify as disabled and to be further protected from discrimination under the ADA. The ADAAA also effectively overturns key rulings in the U.S. Supreme Court cases of Sutton v. United Air Lines, Inc. and Toyota Motor Manufacturing v. Williams. Long-acting reversible contraceptive acceptability and unintended pregnancy among women presenting for short-acting methods: a randomized patient preference trial.

Measures of contraceptive effectiveness combine technology and user-related factors. Observational studies show higher effectiveness of long-acting reversible contraception compared with short-acting reversible contraception. Women who choose long-acting reversible contraception may differ in key ways from women who choose short-acting reversible contraception, and it may be these differences that are responsible for the high effectiveness of long-acting reversible contraception. Wider use of long-acting reversible contraception is recommended, but scientific evidence of acceptability and successful use is lacking in a population that typically opts for short-acting methods. The objective of the study was to reduce bias in measuring contraceptive effectiveness and better isolate the independent role that long-acting reversible contraception has in preventing unintended pregnancy relative to short-acting reversible contraception. We conducted a partially randomized patient preference trial and recruited women aged 18-29 years who were seeking a short-acting method (pills or injectable). Participants who agreed to randomization were assigned to 1 of 2 categories: long-acting reversible contraception or short-acting reversible contraception. Women who declined randomization but agreed to follow-up in the observational cohort chose their preferred method. Under randomization, participants chose a specific method in the category and received it for free, whereas participants in the preference cohort paid for the contraception in their usual fashion. Participants were followed up prospectively to measure primary outcomes of method continuation and unintended pregnancy at 12 months. Kaplan-Meier techniques were used to estimate method continuation probabilities. Intent-to-treat principles were applied after method initiation for comparing incidence of unintended pregnancy. All of the 4000 infants who die daily in India have been bottle fed. Most of these infants die from infections which are typically caused by bottle feeding. Considerable research has shown that human breast milk ideally suits babies' needs. Human breast milk protects infants from several infections and allergies, such that the breastfed infant is 25 times less likely than the bottle fed infant to die due to diarrhea and pneumonia. Comparative studies have even found breastfed babies to have higher IQs than bottle fed ones. Detrimental maternity home practices, adverse social factors, and the unethical and aggressive marketing strategy adopted by the manufacturers of infant milk substitutes and feeding bottles are the major factors responsible for the erosion of the practice of breastfeeding. These factors are discussed. The Infant Milk Substitutes, Feeding Bottles, and Infant Foods Act prohibits the advertisement and promotion of feeding bottles and infant milk substitutes by unethical marketing strategies. Violations of the act are punishable by imprisonment and a heavy fine. The act and the need for its passage are discussed.

This article is a response to Nordén and Norlander's 'Absence of Positive Results for Flexible Assertive Community Treatment. What is the next approach?'[1], in which they assert that 'at present [there is] no evidence for Flexible ACT and… that RACT might be able to provide new impulses and new vitality to the treatment mode of ACT'. We question their analyses and conclusions. We clarify Flexible ACT, referring to the Flexible Assertive Community Treatment Manual (van Veldhuizen, 2013) [2] to rectify misconceptions. We discuss Nordén and Norlander's interpretation of research on Flexible ACT. The fact that too little research has been done and that there are insufficient positive results cannot serve as a reason to propagate RACT. However, the Resource Group method does provide inspiration for working with clients to involve their networks more effectively in Flexible ACT.

Environmental policies are often based on the assumption that people only act environmentally friendly if some extrinsic reward is implicated, usually money. We argue that people might also be motivated by intrinsic rewards: doing the right thing (such as acting environmentally friendly) elicits psychological rewards in the form of positive feelings, a phenomenon known as warm glow. Given the fact that people's psychological state may affect their thermal state, we expected that this warm glow could express itself quite literally: people who act environmentally friendly may perceive the temperature to be higher. In two studies, we found that people who learned they acted environmentally friendly perceived a higher temperature than people who learned they acted environmentally unfriendly. The underlying psychological mechanism pertains to the self-concept: learning you acted environmentally friendly signals to yourself that you are a good person. Together, our studies show that acting environmentally friendly can be psychologically rewarding, suggesting that appealing to intrinsic rewards can be an alternative way to encourage pro-environmental actions.

Staff turnover on assertive community treatment (ACT) teams is a poorly understood phenomenon. This study examined annual turnover and fidelity data collected in a statewide implementation of ACT over a 5-year period. Mean annual staff turnover across all observations was 30.0%. Turnover was negatively correlated with overall fidelity at Year 1 and 3. The team approach fidelity item was negatively correlated with staff turnover at Year 3. For 13 teams with 3 years of follow-up data, turnover rates did not change over time. Most ACT staff turnover rates were comparable or better than other turnover rates reported in the mental health and substance abuse literature. PMID:20012481

Electron paramagnetic resonance (EPR) spectroscopy was used to examine insulins interactions with free radicals. Human recombinant DNA insulins of three groups were studied: short-acting insulin (Insuman Rapid); intermediate-acting insulins (Humulin N, Insuman Basal), and pre-mixed insulins (Humulin M3, Gensulin M50, Gensulin M40, Gensulin M30). The aim of an X-band (9.3GHz) study was comparative analysis of antioxidative properties of the three groups of human insulins. DPPH was used as a stable free radical model. Amplitudes of EPR lines of DPPH as the paramagnetic free radical reference, and DPPH interacting with the individual tested insulins were compared. For all the examined insulins kinetics of their interactions with free radicals up to 60 min were obtained. The strongest interactions with free radicals were observed for the short-acting insulin - Insuman Rapid. The lowest interactions with free radicals were characteristic for intermediate-acting insulin - Insuman Basal. The pre-mixed insulins i.e. Humulin M3 and Gensulin M50 revealed the fastest interactions with free radicals. The short acting, intermediate acting and premixed insulins have been found to be effective agents in reducing free radical formation in vitro and should be further considered as potential useful tools in attenuation of oxidative stress in diabetic patients. The "PHS Increased Risk" Label Is Associated With Nonutilization of Hundreds of Organs per Year.

The Public Health Service "Increased Risk" (PHS IR) designation identifies donors at increased risk of transmitting hepatitis B, C, and human immunodeficiency virus. Although the risk remains very low in the era of nucleic acid testing, we hypothesized that this label may result in decreased organ utilization. Organ Procurement and Transplantation Network data were used to compare utilization rates between PHS-IR and non-PHS-IR donors, as well as to compare export rates and variation in utilization. Among adult standard criteria donors between 2010 and 2013 with a known PHS-IR status, covariate-adjusted utilization rates were lower among PHS-IR donors than non-PHS-IR donors for all organs. For example, 4073 (76.7%) of 5314 PHS-IR kidneys were used, compared with 25 490 (83.7%) of 30 456 non-PHS-IR kidneys-an absolute difference of 7%. Furthermore, all PHS-IR organs had higher export rates than non-PHS-IR organs. For example, 28.7% of PHS-IR kidneys were exported versus 19.7% of non-PHS-IR kidneys. Finally, the utilization rate of PHS-IR organs varied by Donation Service Area; utilization ranged from 20% to 100% among adult kidneys, suggesting significant variation in practices. Similar patterns were seen among pediatric donors. Based on the covariate-adjusted model, if the PHS-IR label did not exist, there could be an additional 313 transplants performed in the United States each year. The PHS "increased risk" label appears to be associated with nonutilization of hundreds of organs per year, despite the very low risk of disease transmission. Better tools are needed to communicate the magnitude of risk to patients and their families.

There are a number of obstacles to increasing the supply of cadaveric organs for transplantation. These include reluctance on the part of relatives to agree to the so called harvesting of organs from their deceased relative, and the unwillingness of some doctors to approach grieving families and ask consent for this harvesting to take place. In this paper I will focus on the altruistic act of asking that the latter entails, and will argue that failure to acknowledge the personal cost of this act to physicians is having an adverse impact on the supply of organs. I will draw analogies with the almost equally neglected altruistic act of undertaking anatomy dissection and all of the related breaking of societal taboos. I will examine the language used in discussions about increasing organ supply and conclude that the terms cadaveric and harvest are unhelpful in gaining public confidence. A process and vocabulary that openly acknowledges and validates the altruistic acts demanded of all the human beings involved--donors, recipients, their respective relatives, and the health professionals who mediate between them--is needed if the supply of organs is to be increased.

Effects of Acceptance and Commitment Therapy (ACT) Training on Clinical Psychology Trainee Stress, Therapist Skills and Attributes, and ACT Processes.

Despite the increasing uptake of Acceptance and Commitment Therapy (ACT) by mental health practitioners, few studies have investigated the effects of ACT training on trainees. Clinical psychology trainees (CPTs) are susceptible to high stress such that their training represents a teachable moment for personal application of the therapy skills they learn for clinical practice. This study investigates the effects of ACT training on stress, therapist skills and attributes, and the personal acquisition of ACT strategies in CPTs. Thirty-two CPTs completed questionnaires before and after university-based ACT training that consisted of 12 2-h weekly workshops. Pairwise t-tests showed that CPTs reported improvements from before to after training on measures of counselling self-efficacy, client-therapist alliance, self-kindness, acceptance, defusion, mindfulness and values, and a marginally significant improvement on somatic symptoms, despite a trend towards increased work-related stress. As predicted, each of the ACT process variables was related to one or more of the therapist stress, skill and attribute variables, such that greater levels of mindfulness, values and acceptance, and less thought suppression were related to better trainee outcomes. This study provides preliminary data on therapist skill development and personal benefits for CPTs related to receiving ACT training that interweaves instruction in competencies acquisition with self-care. The traditional focus of work on personality and behavior has tended toward "major outcomes" such as health or antisocial behavior, or small sets of behaviors observable over short periods in laboratories or in convenience samples. In a community sample, we examined a wide set (400) of mundane, incidental or "every day" behavioral acts, the frequencies of which were reported over the past year. Using an exploratory methodology similar to genomic approaches (relying on the False Discovery Rate) revealed 26 prototypical acts for Intellect, 24 acts for Extraversion, 13 for Emotional Stability, nine for Conscientiousness, and six for Agreeableness. Many links were consistent with general intuition-for instance, low Conscientiousness with work and procrastination. Some of the most robust associations, however, were for acts too specific for a priori hypothesis. For instance, Extraversion was strongly associated with telling dirty jokes, Intellect with "loung[ing] around [the] house without clothes on", and Agreeableness with singing in the shower. Frequency categories for these acts changed with markedly non-linearity across Big Five Z-scores. Findings may help ground trait scores in emblematic acts, and enrich understanding of mundane or common behavioral signatures of the Big Five.

Now in its ninth year of operations, the Advanced Communications Technology Satellite (ACTS) program has continued, although since May 2000 in a new operations arrangement involving a university based consortium, the Ohio Consortium for Advanced Communications Technology (OCACT), While NASA has concluded its experimental intentions of ACTS, the spacecraft's ongoing viability has permitted its further operations to provide educational opportunities to engineering and communications students interested in satellite operations, as well as a Ka-band test bed for commercial interests in utilizing Kaband space communications. The consortium has reached its first year of operations. This generous opportunity by NASA has already resulted in unique educational opportunities for students in obtaining "hands-on" experience, such as, in satellite attitude control. An update is presented on the spacecraft and consortium operations.

Current HIV treatment options require daily use of combination antiretroviral drugs. Many persons living with HIV experience treatment fatigue and suboptimal adherence as a result. Long-acting antiretroviral drugs are being developed to expand options for HIV treatment. Here, we review the agents in development, and evaluate data from recent clinical trials. In addition, we anticipate challenges to successful widespread use of long-acting antiretrovirals. Parenteral nanosuspensions of cabotegravir and rilpivirine, and dapivirine vaginal ring are the farthest in clinical development. Long-acting modalities in earlier development stages employ drug-loaded implants, microparticles, or targeted mutagenesis, among other innovations. Long-acting antiretroviral drugs promise new options for HIV prevention and treatment, and ways to address poor adherence and treatment fatigue. Further studies will identify the long-acting agents or combinations that are suitable for routine use. Creative solutions will be needed for anticipated implementation challenges.

The IntAct website has been improved to enhance the search process and in particular the graphical display of the results. Self-acting seals are described in detail. The mathematical models for obtaining a seal force balance and the equilibrium operating film thickness are outlined. Particular attention is given to primary ring response (seal vibration) to rotating seat face runout. This response analysis reveals three different vibration models with secondary seal friction being an important parameter. Leakage flow inlet pressure drop and affects of axisymmetric sealing face deformations are discussed. This article is a response to Nordén and Norlander’s ‘Absence of Positive Results for Flexible Assertive Community Treatment. What is the next approach?’[1], in which they assert that ‘at present [there is] no evidence for Flexible ACT and… that RACT might be able to provide new impulses and new vitality to the treatment mode of ACT’. We question their analyses and conclusions. We clarify Flexible ACT, referring to the Flexible Assertive Community Treatment Manual (van Veldhuizen, 2013) [2] to rectify misconceptions. We discuss Nordén and Norlander’s interpretation of research on Flexible ACT. The fact that too little research has been done and that there are insufficient positive results cannot serve as a reason to propagate RACT. Although reauthorization of the Clean Water Act was scheduled for 1992, action has stalled due to debate over the contentious wetland issue and is not likely to pick up again until after the first of the year. The subject of the debate, regulation of wetlands, is found in Section 404, which makes up only a small part of the Clean Water Act.At the meeting, “New Directions in Clean Water Policy,” held in Charlottesville, Va., July 28-31, Ralph Heimlich, an economist with the Office of Policy and Planning Evaluation at the Environmental Protection Agency, said “it is unfortunate that the Section 404 Dredge and Fill permit program, our only national wetland regulation, is part of the Clean Water Act.” The controversy over wetland reform, he added, “has delayed and threatens to poison action on Clean Water Act reauthorization, provisions that may be more directly significant in improving water quality.”

Despite their widespread use, long acting antipsychotics, are often regarded with prejudice, due to fears of punishment, control and insufficient evolution towards psychosocial development of psychotic patients raised by their improper utilization. Another major shortcoming of long-acting antipsychotics is the impossibility of altering their dosage if side-effects appear. However, long-acting antipsychotics proved effective in schizophrenia and other severe psychotic disorders as a consequence of stable dose administration, leading to reduction of relapses and increased treatment adherence. Therapeutic opportunities have also risen after introduction of newer long acting second generation antipsychotics in recent years. Newer long-acting antipsychotics were developed to tackle the need for pharmacotherapy enhancing adherence in integrated rehabilitation programmes. This review is an outline of the development and introduction of older and newer long-acting antipsychotics in the treatment of schizophrenia and other psychoses, with considerations on past and present pharmacological and therapeutic issues.

To examine trends in the source of modern contraception (public versus private sector); method choice (long-acting or permanent methods versus short-acting methods); and method and source combined. A retrospective analysis was conducted using data collected by national Demographic and Health Surveys and Reproductive Health Surveys during the period 1992-2012. The dataset included 18 low-income countries in Sub-Saharan Africa, 10 from Latin America and the Caribbean (LAC), and 8 from Asia. A substantial proportion-between 40% and 49%-of modern contraceptive users relied on the private sector in Asia and LAC in the last 20years, yet the proportion has been smaller in Sub-Saharan Africa, between 27% and 30%. Increased use of short-acting methods from both public and private sectors has driven the rise in contraceptive prevalence in Asia and LAC. Similarly, increased contraceptive prevalence in Sub-Saharan Africa reflected the increased use of short-acting methods obtained mainly through the public sector, with only limited use of long-acting or permanent methods through the private sector. The private sector has played a key role in the increase of modern CPR and the provision of modern contraceptives around the world, providing almost half of them in low-income countries. Yet, such increase was driven primarily by a more substantial role in the provision of short-acting methods than long acting and permanent methods. Crown Copyright © 2015. Self-acting and hydrodynamic seals are described. The analytical procedures are outlined for obtaining a seal force balance and the operating film thickness. Particular attention is given to primary ring response (seal vibration) to rotating seat face runout. This response analysis revealed three different vibration modes. Proposed applications of self-acting seals in gas turbine engines and in rocket vehicle turbopumps are described. Also experimental data on self-acting face seals operating under simulated gas turbine conditions are given; these data show the feasibility of operating the seal at conditions of 345 newtons per square centimeter (500 psi) and 152 meters per second (500 ft/sec) sliding speed.

Commercialization of Advanced Communications Technology Satellite (ACTS) technology

In an on-going effort to maintain United States leadership in communication satellite technology, the National Aeronautics and Space Administration (NASA), led the development of the Advanced Communications Technology Satellite (ACTS). NASA's ACTS program provides industry, academia, and government agencies the opportunity to perform both technology and telecommunication service experiments with a leading-edge communication satellite system. Over 80 organizations are using ACTS as a multi server test bed to establish communication technologies and services of the future. ACTS was designed to provide demand assigned multiple access (DAMA) digital communications with a minimum switchable circuit bandwidth of 64 Kbps, and a maximum channel bandwidth of 900 MHZ. It can, therefore, provide service to thin routes as well as connect fiber backbones in supercomputer networks, across oceans, or restore full communications in the event of national or manmade disaster. Service can also be provided to terrestrial and airborne mobile users. Commercial applications of ACTS technologies include: telemedicine; distance education; Department of Defense operations; mobile communications, aeronautical applications, terrestrial applications, and disaster recovery. This paper briefly describes the ACTS system and the enabling technologies employed by ACTS including Ka-band hopping spot beams, on-board routing and switching, and rain fade compensation. When used in conjunction with a time division multiple access (TDMA) architecture, these technologies provide a higher capacity, lower cost satellite system. Furthermore, examples of completed user experiments, future experiments, and plans of organizations to commercialize ACTS technology in their own future offerings will be discussed.
