  1. Proteins with molecular masses of 25 to 40 kilodaltons elicit optimal protective responses against Plasmodium chabaudi adami infection.

    The presence of the CD4+ T cell has been shown to be crucial for resolution of acute infection in the Plasmodium chabaudi adami murine malaria model. This model is, therefore, suitable for the isolation of malaria antigens that are capable of activating protective T cells. In light of this, we set out to identify P. chabaudi adami molecules that activate protective responses in this model. Denatured P. chabaudi adami proteins were isolated by continuous-flow electrophoresis on the basis of their apparent molecular masses and then sequentially assessed for the ability to protect mice in immunization experiments. We report here that low-molecular-mass P. chabaudi adami polypeptides in the range from 25 to 40 kDa are most effective at immunizing mice against a challenge infection with viable P. chabaudi adami. The method used to obtain these proteins could also be applied to identify molecules that activate protective cell-mediated responses in other infectious disease models.

  2. Induction of strain-transcending immunity against Plasmodium chabaudi adami malaria with a multiepitope DNA vaccine.

    A major goal of current malaria vaccine programs is to develop multivalent vaccines that will protect humans against the many heterologous malaria strains that circulate in endemic areas. We describe a multiepitope DNA vaccine, derived from a genomic Plasmodium chabaudi adami DS DNA expression library of 30,000 plasmids, which induces strain-transcending immunity in mice against challenge with P. c. adami DK. Segregation of this library and DNA sequence analysis identified vaccine subpools encoding open reading frames (ORFs)/peptides of >9 amino acids [aa] (the V9+ pool, 303 plasmids) and >50 aa (V50+ pool, 56 plasmids), respectively. The V9+ and V50+ plasmid vaccine subpools significantly cross-protected mice against heterologous P. c. adami DK challenge, and protection correlated with the induction of both specific gamma interferon production by splenic cells and opsonizing antibodies. Bioinformatic analysis showed that 22 of the V50+ ORFs were polypeptides conserved among three or more Plasmodium spp., 13 of which are predicted hypothetical proteins. Twenty-nine of these ORFs are orthologues of predicted Plasmodium falciparum sequences known to be expressed in the blood stage, suggesting that this vaccine pool encodes multiple blood-stage antigens. The results have implications for malaria vaccine design by providing proof-of-principle that significant strain-transcending immunity can be induced using multiepitope blood-stage DNA vaccines and suggest that both cellular responses and opsonizing antibodies are necessary for optimal protection against P. c. adami.

    A new species of apseudomorph tanaidacean, Apseudopsis adami sp. nov., is described, including intraspecific variation, from specimens collected in the seagrass meadows of O Grove inlet (NW Iberian Peninsula, NE Atlantic). The new species is characterized by the unique combination of the following characters: pointed rostrum with rounded shoulders, four ventral propodal spines on pereopod 1, and posterolateral pointed apophyses on pereonites 2-5, regardless of the developmental stage of the individual. Two juvenile stages were identified before reaching maturity; early juvenile limbs are scarcely ornamented in comparison to the adults. Advanced juveniles represent the vast majority of the studied population, and the limb ornamentation is similar to that of the adults. Males show two different cheliped morphologies. After a preparatory stage, females pass through a sequence of copulatory instars followed by intermediate stages in which the female loses the ovisac after manca release. The number of segments of the antennular and antennal flagella and uropod endopods, as well as the development and ornamentation of the mandibular palp, are meristic changes that occur during development and allow rapid identification of the developmental instar.

  5. Juxtaposition of Neoproterozoic units along the Baruda - Tulu Dimtu shear-belt in the East African Orogen of western Ethiopia

    Amalgamation of East and West Gondwanaland during the Neoproterozoic East African Orogen is recorded by several shear-belts or 'suture zones', some of which are associated with ultramafic and mafic complexes that have been interpreted as ophiolite fragments. The Baruda shear-belt is a major structure of this type that belongs to the N-S trending Barka - Tulu Dimtu zone. The significance of this zone has been studied within a transect in western Ethiopia which covers a variety of metasedimentary and metavolcanic sequences, ultramafic rocks and synkinematic intrusive complexes. All rocks participated in the regional D1 event as reflected in a penetrative steep foliation in supracrustal rocks and marginal parts of the intrusions. Highly strained rocks contain a stretching lineation that plunge to the east. The several-km thick Baruda shear-belt, comprising mylonitic supracrustal and plutonic rocks including mafic-ultramafic mega-lenses, is the most prominent expression of this event. Shear-sense indicators demonstrate top-to-the-west shear. Subsequent D2 deformation is recorded in 2-300 m wide, N-S striking, subvertical shear-zones with subhorizontal stretching lineation relatable to sinistral transcurrent movements. Our data indicate that rock units on either side of the Baruda shear-belt are related, rather than being exotic to each other as implied in suture zone models, since there is no major lithologic or metamorphic difference, geochemical data on metavolcanic rocks and pre-tectonic intrusions suggest a paleotectonic link, and style and extent of deformation is similar across the shear-belt. A tentative model for the transect suggests an arc and back-arc setting which experienced later continental collision and tectonic shortening. The initial setting was that of a shallow marine platform characterised by carbonates and sandstones, which covered extensive areas prior to break-up of a pre-existing supercontinent.     The enhancement of immunogenicity of malarial DNA vaccines is important if they are to have practical application in protecting against blood-stage malaria. Here we describe three different DNA vaccine vector types used in conjunction with the blood-stage merozoite surface protein 4/5 (MSP4/5), the murine homologue of Plasmodium falciparum MSP4 and MSP5, in an attempt to enhance survival against lethal Plasmodium chabaudi adami DS blood-stage challenge. MSP4/5 was inserted into VR1020 (secretory), monocyte-chemotactic protein-3 (MCP-3) (chemoattractant), and cytotoxic T-lymphocyte antigen 4 (CTLA4) (lymph node targeting) vectors. Mice were immunized intradermally via gene-gun, IM injection, or boosting with recombinant MSP4/5 protein. Antibody responses after boosting were predominantly of the IgG1 and IgE isotypes, with low avidity antibodies produced in DNA primed groups. Despite antibody responses comparable to recombinant protein immunization, boosting mice primed with antigens encoded by MCP-3 and CTLA4 vectors did not enhance survival compared to vector control groups. Gene-gun vaccination using VR1020/MSP4/5 followed by recombinant MSP4/5 boosting, or gene-gun DNA vaccination alone using MCP-3/MSP4/5, resulted in enhanced survival compared to empty vector control mice. The results suggest that the enhancement of survival against lethal blood-stage malaria challenge after utilizing MSP4/5 DNA vaccination is therefore highly dependent on the route and type of vaccine vector employed.

    This paper will present the status of the development of district cooling systems in Scandinavia over the last 5 years. It will describe the technologies used in the systems that have been constructed as well as the options considered in different locations. It will identify the drivers for the development of the cooling business to-date, and what future drivers for a continuing development of district cooling in Sweden. To-date, approximately 25 different cities of varying sizes have completed feasibility studies to determine if district cooling is an attractive option. In a survey, that was conducted by the Swedish District Heating Association, some 25 cities expected to have district cooling systems in place by the year 2000. In Sweden, district heating systems with hot water is very common. In many cases, it is simply an addition to the current service for the district heating company to also supply district cooling to the building owners. A parallel from this can be drawn to North America where district cooling systems now are developing rapidly. I am convinced that in these cities a district heating service will be added as a natural expansion of the district cooling company`s service.

    District nurses constitute the basis of the primary health care services in Dominica. All encounters of three district nurses were registered using the international classification of primary care. Information on other aspects of district nursing was collected by participating observation and the use of a questionnaire. Check-ups for hypertension, diabetes, pregnancy and immunisations constituted 40% of all reasons for encounter. The district nurses dealt with 80% of all contacts; only 20% of all patients were referred to the district medical officer. There are several discouragements to the motivation of the nurses. In addition to being a nurse, all have their family and other obligations. Postgraduate training with diversified certification and upgrading of wages could contribute to a continued high motivation and increased job satisfaction.

    Pribram is one of the largest and richest vein uranium districts in the world. The Pribram district has accounted for about 60 percent of Czechoslovakia`s total uranium production. The Pribram uranium district is located about 60 kilometers southwest of Prague, in Cezechslovakia`s central Bohemia region. This district contains perigranitic, monometallic, vein-type uranium deposits. The deposits are within a northeast-southwest elongated area, about 20 kilometers long and 1-2 kilometers wide, located between Oboriste in the northeast and Tresko in the southwest. Several thousand veins have been discovered; about 1,600 have been mined. Most of the veins are grouped in clusters, which are intense accumulations of veins paralleling or intersecting each other within a narrow segment. Until this year, all uranium production was exported to the USSR, with only the amount required for Czechoslovakia`s nuclear power stations being returned (as fabricated fuel). Most of Czechoslovakia`s present and future uranium production will come from sandstone deposits in the North Bohemian Cretaceous Basin, such as Hamr and Straz.

    As changes in society and health provision mean that one in four people over the age of 75 will require nursing care at home, pre-registration adult nurse education increasingly prepares student nurses for a future career within the community. District nurses undertake complex, multidimensional health and social assessments and care in a non-clinical setting and work in partnership with patients and their significant others to promote practical and psychological coping mechanisms and self-care. The district nurse's first assessment visit is key to developing a therapeutic partnership and it is often during this visit that expertise in district nursing practice emerges. The holistic, contextual and dynamic aspects of nursing in the home setting can make district nursing expertise difficult to illustrate and demonstrate within the classroom setting. This article explores the ways in which an understanding of expertise development theory can enable the tacit expertise that occurs within the first assessment visit to be made visible to student nurses, using simulation and expert narrative as a pedagogical strategy.     11 million residents in 1450 square kilometers make the Calcutta Metropolitan District the world's most densely packed metropolis and the world's 6th largest urban agglomeration. But even though Calcutta is India's largest city, it is growing at a much slower pace than other Indian cities. Its annual growth rate between 1971 and 1981 was 2.65%, well below the 3.8% growth rate for India's urban population as a whole. Even at this relatively slow growth rate, however, Calcutta's population will still grow to 11.7 million residents in 1990 and 15.9 million in 2000. Calcutta's failure to create urban jobs quickly enough to accommodate its vast population increase has led to widespread evidence of unemployment and extreme poverty. Many in Calcutta complain that the central goverment has thwarted development and international aid to Calcutta. Industrial stagnation has slowed the area's urbanization and rural-urban migration. As greater numbers of new job seekers enter the labor force and the dropout rate diminishes due to dramatic inprovement in health, relentless pressure is put on Calcutta's already strained economy. Calcutta's job seekers will be partly absorbed by the informal sector; one study estimates that 40-50% of Calcutta's labor force is employed in the informal sector. In 1971, 6% of Calcutta's work force was employed in agriculture, 40% in manufacturing, and 54% in services. 2/3 of the population make less than $35 a month, and about 10% are officially unemployed. Despite great improvements in public works, Calcutta's slums are still India's worst. Living standards have gone down compared to India as a whole. Most of the middle class has moved to the suburbs; what is left in the central core is the rich and the poor. However, despite widening income disparities, Calcutta is still a peaceful city--especially so at a time when India is marked with so much violence.

    The feasibility of developing a demonstration cogeneration hot water district heating system was studied. The district heating system would use coal and cogenerated heat from the Moorhead power plant to heat the water that would be distributed through underground pipes to customers or their space and domestic water heating needs, serving a substantial portion of the commercial and institutional loads as well as single and multiple family residences near the distribution lines. The technical feasibility effort considered the distribution network, retrofit of the power plant, and conversion of heating systems in customers' buildings to use hot water from the system. The system would be developed over six years. The economic analysis consisted of a market assessment and development of business plans for construction and operation of the system. Rate design methodology, institutional issues, development risk, and the proposal for implementation are discussed.

    The aim of the Political Districting Problem is to partition a territory into electoral districts subject to some constraints such as contiguity, population equality, etc. In this paper, we apply statistical physics methods to Political Districting Problem. We will show how to transform the political problem to a spin system, and how to write down a q-state Potts model-like energy function in which the political constraints can be written as interactions between sites or external fields acting on the system. Districting into q voter districts is equivalent to finding the ground state of this q-state Potts model. Searching for the ground state becomes an optimization problem, where optimization algorithms such as the simulated annealing method and Genetic Algorithm can be employed here.

    A study of neonatal mortality in Meerut district revealed an infant mortality rate of 50.1 per 1000 live births. Neonatal mortality accounted for 37.8% of infant mortality with a neonatal mortality rate of 19.0 per 1000 live births. 90.5% of these neonates were delivered at home largely by untrained personnel (57.2%). Only 28.6% of these neonates were treated by qualified doctors and only 30.9% of their mothers were fully immunized against tetanus. At least 2/3rd of neonatal mortality was due to exogenous factors with tetanus neonatorum and septicaemia being the principal causes of mortality each accounting for a mortality rate of 4.7 per 1000 live births.     The US Department of Housing and Urban Development (HUD) and the US Department of Energy (DOE) cosponsor a program aimed at increasing the number of district heating and cooling systems. Twenty-eight communities have received HUD cooperative agreements to aid in a national feasibility assessment of district heating and cooling systems. The HUD/DOE program includes technical assistance provided by Argonne National Laboratory and Oak Ridge National Laboratory. Part of this assistance is a computer program, called the district heating strategy model, that performs preliminary calculations to analyze potential district heating and cooling systems. The model uses information about a community's physical characteristics, current electricity-supply systems, and local economic conditions to calculate heat demands, heat supplies from existing power plants and a new boiler, system construction costs, basic financial forecasts, and changes in air-pollutant emissions resulting from installation of a district heating and cooling system. This report explains the operation of the district heating strategy model, provides simplified forms for organizing the input data required, and describes and illustrates the model's output data. The report is written for three groups of people: (1) those in the HUD/DOE-sponsored communities who will be collecting input data, and studying output data, to assess the potential for district heating and cooling applications in their communiites; (2) those in any other communities who may wish to use the model for the same purpose; and (3) technical-support people assigned by the national laboratories to explain to community personnel how the model is used.

    A survey commissioned by the Patients Association (2015) has shown that the risky practice of altering solid-form medication to assist patients with swallowing difficulties is a daily occurrence in care homes. District nurses and community matrons who provide care to patients and assist staff in care homes are well-placed to raise awareness of the impact that tampering with tablets has on patient safety. It is essential that district nurses and community matrons discharge their duty of care to patients in care homes by supporting effective medication management that meets the individual needs of those patients. This article considers the legal and professional standards that must inform a district nurse's support of medication management for a patient with swallowing difficulties residing in a care home.     District nurse (DN) care delivery has undergone substantial change in recent years due to changing demographics and service delivery demands that have called for a move of care delivery from secondary to primary care. The title District Nurse is recorded with the Nursing and Midwifery Council (NMC) on completion of the Specialist Practice Qualification in District Nursing (SPQ DN), which purports to be a 'transformational' course that prepares future caseload holders to manage their team and prioritise care delivery effectively. This article explores the need for assertiveness skills in this role in response to Australian research, and outlines the pedagogic interventions implemented during the SPQ DN course to enhance this skill. Assertiveness scores were monitored for the duration of the course and demonstrated a significant increase-a topic that is now the subject of a future, funded study. PMID:27479854

The author critically reviews various attempts to explain spirit possession as solely a psychological or sociological event. Instead, he turns to the region's spirit possession and mediumship cults, oral traditions and social ideology for an ethnographically relevant interpretation of spirit possession.

  12. Understanding the organisational culture of district health services: Mahalapye and Ngamiland health districts of Botswana

    Background Botswana has a shortage of health care workers, especially in primary health care. Retention and high performance of employees are closely linked to job satisfaction and motivation, which are both highest where employees’ personal values and goals are realised. Aim The aim of the study was to evaluate employees’ personal values, and the current and desired organisational culture of the district health services as experienced by the primary health care workers. Setting The study was conducted in the Ngamiland and Mahalapye health districts. Method This was a cross sectional survey. The participants were asked to select 10 values that best described their personal, current organisational and desired organisational values from a predetermined list. Results Sixty and 67 health care workers completed the survey in Mahalapye and Ngamiland districts, respectively. The top 10 prevalent organisational values experienced in both districts were: teamwork, patient satisfaction, blame, confusion, job insecurity, not sharing information and manipulation. When all the current values were assessed, 32% (Mahalapye) and 36% (Ngamiland) selected by health care workers were potentially limiting organisational effectiveness. The organisational values desired by health care workers in both districts were: transparency, professional growth, staff recognition, shared decision-making, accountability, productivity, leadership development and teamwork. Conclusions The experience of the primary health care workers in the two health districts were overwhelmingly negative, which is likely to contribute to low levels of motivation, job satisfaction, productivity and high attrition rates. There is therefore urgent need for organisational transformation with a focus on staff experience and leadership development. PMID:26842516

    The dissemination of information at the district level in developing countries is hampered by an acute shortage of teaching materials and health information and the lack of libraries accessible to potential users. The lack of libraries is particularly serious, because of their usefulness in continuing education, job training, and supportive supervision. Health workers are helped by libraries in rural areas where professional support is lacking. Health workers in rural areas could serve to spread information on primary care and as literature resources for planners and evaluators of health services, and to facilitate the transmission of information from the system in which they operate. It is important that basic training be reinforced with updated health information; sophisticated systems of communication may not be affordable in remote or rural areas. A pilot project to address these concerns was developed by the Instituto Superiore di Sanita in Rome; the project would establish 30 district health libraries in Uganda and the Tanzania. The libraries would provide materials relevant to the users' local work activities, working environments, learning styles, language and semantics, and complementary to other available materials. Selection of materials would take place in consultation with local personnel. Questionnaires were administered to 30 district medical officers in Ethiopia, Tanzania, and Uganda; included in the survey were questions about their recommendations for 25 essential books and 20 books selected from the World Health Organization and Teaching Aids at Low Cost lists. A standard package of materials was prepared on clinical medicine, preventive health measures, primary care management, and other germane topics, and delivered to the target countries in July and August, 1992. Simultaneously coordinated workshops were held to introduce the packages to district medical officers, discuss the role of libraries in the continuing education of health workers

    The district, regional or state laboratory is the local laboratory to which veterinarian practitioners usually submit samples, and consequently these laboratories are usually the first to observe a suspected disease problem. In most countries, these laboratories are under the jurisdiction of the State or region in which they are located. In the United States of America (USA), most veterinary diagnostic laboratories are State-associated and operate under the aegis of either the State Department of Agriculture or a university. The national laboratory provides reference assistance to the State laboratories. In the USA, the national Laboratory (the National Veterinary Services Laboratories) acts as a consultant to confirm difficult diagnoses and administer performance tests for State-associated laboratories. District, state or regional laboratories need to share information regarding technological advances in diagnostic procedures. This need was met in the USA by the formation of the American Association of Veterinary Laboratory Diagnosticians (AAVLD) in the late 1950s. Another requirement of district, state or regional diagnostic laboratories is a method to confirm quality assurance, which was fulfilled in the USA by an accreditation programme established through the AAVLD. The Accreditation Committee evaluates laboratories (on request) in terms of organisation, personnel, physical facilities and equipment, records, finance and budget. Those laboratories which meet the standards as established in the 'Essential Requirements for Accreditation' are given accreditation status, which indicates that they have the expertise and facilities to perform tests on food-producing animals for shipment in national or international commerce and on companion, laboratory or zoo animals. While confidentiality of test records is most important, it is becoming necessary to release certain types of animal disease test information if a country is to participate in the exportation of animals
