    Secretary of the Interior Ken Salazar is one of the latest members of the Obama administration to announce that he is leaving his position near the start of President Obama's second term in office. Salazar, who has served as interior secretary since January 2009, intends to leave the department by the end of March, the department noted on 16 January. Salazar joins a number of other key officials who are planning to leave the administration. They include Environmental Protection Agency administrator Lisa Jackson, National Oceanic and Atmospheric Administration administrator Jane Lubchenco, and U.S. Geological Survey director Marcia McNutt.

    The approach of the new year is a traditional time to tally up successes, failures, and the path ahead. Jane Lubchenco, administrator of the U.S. National Oceanic and Atmospheric Administration (NOAA), examined some agency advances and significant challenges during the 7 December Union Agency Lecture at the AGU Fall Meeting, during a press briefing, and in an interview with Eos. Lubchenco focused on several key areas including the concern about monitoring, mitigating, and managing extreme events; budgetary pressures the agency faces in current fiscal year (FY) 2012 and in FY 2013, with President Barack Obama on 18 November having signed into law a bill, HR 2112, following congressional agreement on a budget legislation conference report; and NOAA's newly released scientific integrity policy (see "NOAA issues scientific integrity policy," Eos Trans. AGU, 92(50), 467, doi:10.1029/2011EO500004, 2011).

    In the early 1980s, the leadership of the antiabortion movement became involved in a campaign to establish legal rights to extraordinary medical care for seriously handicapped newborns. Armed with political contacts in the Reagan administration and Congress, and allied with advocates for the disabled, the antiabortion movement searched for a test case to guide through the courts. Antiabortion advocate Lawrence Washburn found such a case in Baby Jane Doe, who was being treated at Stony Brook Medical Center. The movement went on to amend the Child Abuse Act to include protections for handicapped newborns. Activists in the movement chose the issue of Baby Jane Doe because they believed it would attract welcome publicity, give them the appearance of supporting civil rights, and enhance their argument as to the legal rights of the fetus and thus strengthen the case against abortion. The movement was partially successful in obtaining its goals.

  9. Roe v. Wade. Jane's perspective.

    The US Supreme Court's Roe vs. Wade decision was greeted with mixed emotions by women who had been providing illegal abortions through the Chicago Women's Liberation Abortion Service. Originally founded in 1969 to help women find competent underground abortion providers, the members of "Jane," as the organization was known, soon learned the necessary skills to provide abortions themselves. They were compelled to take over the abortion process to lower the cost of abortion and because the process is fairly straightforward and easy to learn. At the time, most people still treated physicians as gods, and women were taught to remain largely ignorant of their bodies. Jane attempted to challenge both medical authority and the repression of women's knowledge by basing its services on respect for woman as decision makers involved in an empowering experience. Roe vs. Wade addressed the surface issue involved in abortion and undermined Jane's attempts to radically alter the power dynamics in medical practice. The court decision placed the responsibility for abortion in the hands of the medical establishment by stating that abortion "is inherently a medical decision." Thus, the members of Jane felt great relief that they could stop placing themselves at risk by operating outside of institutional and societal sanctions, but they also felt that Roe abandoned women to insensitive and disempowering medical treatment. The members of Jane wanted a revolution and got a reform. They wanted no laws on abortion, no state control over women's bodies. Today, abortion is available only in 17% of US counties and is out of reach for low-income women. Other obstacles to abortion accessibility include mandatory waiting periods, parental consent laws, mandatory state-scripted counseling, and a dearth of physicians willing to perform abortions. Roe stands as the law of the land on the backs of poor, young, and rural women.

    Today, no other classic novelist has the popularity or power of Jane Austen, and in 2013 the world will celebrate 200 years of her comic masterpiece Pride and Prejudice. Her millions of fans have an abiding fascination with all aspects of her life, including her health and the cause of her death. This historical ophthalmic study of Jane Austen, based on very incomplete medical bibliographic data, finds that she had a mild ocular surface disorder from age 23. This disorder did not significantly impact on her visual performance for writing. There are many references to eyes in her novels, but Jane's eyes and those of her characters cannot contribute further to the debate around the cause of her death at age 41.

    This essay notes initially recent prominence of theories of pedagogy that attempt to "de-mystify" it and reveal troubling power relations, and their subsequent contention that love is impossible in the student-teacher relationship. "Pedagogical" interpretations of Jane Austen's fiction, however, see pedagogy as essential to love. I argue that this is so precisely because of the power dynamics involved; drawing on Jessica Benjamin's psychoanalytic interpretation of G. W. F. Hegel's analysis of the "Lord-Bondsman," I suggest that Austen portrays the loving relationship as inherently involving the occupation and subsequent exchange of roles as superior and inferior, incarnated as "teacher" and "student."

  3. Jane Austen and Addison's disease: an unconvincing diagnosis.

    Jane Austen's letters describe a two-year deterioration into bed-ridden exhaustion, with unusual colouring, bilious attacks and rheumatic pains. In 1964, Zachary Cope postulated tubercular Addison's to explain her symptoms and her relatively pain-free illness. Literary scholars later countered this posthumous diagnosis on grounds that are not well substantiated, while medical authors supported his conclusion. Important symptoms reported by contemporary Addison's patients-mental confusion, generalised pain and suffering, weight loss and anorexia-are absent from Jane Austen's letters. Thus, by listening to the patient's perspective, we can conclude it is unlikely that Addison's disease caused Jane Austen's demise. Disseminated bovine tuberculosis would offer a coherent explanation for her symptoms, so that Cope's original suggestion of infective tuberculosis as the cause of her illness may have been correct.

Austen's era, which we customarily identify as the time of a romantic return to nature, was marked by conflicts over what nature did and did not include—conflicts between, in the parlance of the period, "the botanist" and "the florist," between an account of nature as that which was given and an account of nature as that which (as with the modern nurseryman's new hybrids and luxuriants) might be manufactured. In arranging for her young heroines' stories of growing up to unfold in the "artificial climates" of modern fashionable gardening and amidst "florists' flowers," Austen deliberately works through those conflicts. She inhabits the plot of "natural development" to which she is often linked in elusively oppositional ways. PMID:21114061

  20. "Typhoid Mary" and "HIV Jane": responsibility, agency and disease prevention.

    The construction of disease risks as knowable, calculable and preventable in dominant social science and public health discourses has fostered a certain kind of logic about individual risk and the responsibility for infection. Disease control measures that have developed out of this logic typically fail to recognise the socio-structural roots of many high-risk behaviours that are linked to the spread of infection. Instead, they hold the disease carrier responsible for managing his/her own risk of infection of others, and rely on constraining the agency of the carrier (e.g. by constraining movement, contact or occupation). In occupations associated with a high risk of infection, the idea of responsibility of the actor implicitly raises issues of "professional responsibility". Using the case of "Typhoid Mary" and a hypothetical case of "HIV Jane", this paper explores some of the problems with making sex workers responsible for the prevention of HIV transmission. It argues that for the notion of "responsibility" to make any sense, the HIV-positive person must be in a position to exercise responsibility, and for this they must have agency.

    Charlotte Brontë's Jane Eyre contains a paradox of character: Jane Eyre is an assertive successful woman, who marries, after every obstacle is overcome, the man she loves; she is also a traumatised, terrorised child, who later becomes deeply involved, to the point of losing herself, with a moral sadist. In the critical literature on Jane Eyre there is controversy concerning Brontë's aims in the characterisation of her fictional autobiographer. Did Brontë's artistic control of character slip and result in Jane Eyre's irreconciliable attributes? Did she mean to give us an assertive feminine heroine and instead reveal her own castration complex in the marriage to a maimed and near-blind man? The hypothesis of this paper is that the multiple functions of masochism--adaptive, defensive and gratifying--the multiple defence mechanisms at work in masochism--denial, introjection and projection--and the special layering of masochism at each developmental level--oral, anal, and phallic--are mirrored in Brontë's characterisation. The irreducible complexity of the clinical concept of masochism finds its match in Brontë's characterisation of Jane Eyre.

    The fall meeting of the National Oceanic and Atmospheric Administration's (NOAA) Science Advisory Board was in part a study in contrasts: discussing the agency's vision, goals, and recent successes while facing the harsh economic and political landscape that will make it difficult for NOAA to receive sufficient funding for the current fiscal year (FY 2011) to do little more than tread water toward reaching some of those goals. During a 30 November presentation, NOAA administrator Jane Lubchenco provided an overview of NOAA's Next Generation Strategic Plan. The document focuses on four long-term goals: climate adaptation and mitigation, a weather-ready nation, resilient coastal communities and economies, and healthy oceans.

  17. Jane Cooke Wright (1919-2013): Pioneering oncologist, woman and humanitarian.

    Jane Wright was a fundamental researcher in cancer chemotherapy in the 1950s-1980s and was one of the first scientists to test anti-cancer drugs on humans rather than solely on mice, discovering the use of the popular antimetabolite drug methotrexate on solid tumours. From her research she was able to conclude which specific anti-cancer agents would have the greatest lethal effect on a patient's certain cancer type and she invented a method of delivering chemotherapy agents directly to an internal cancer site. During a time when the Civil Rights in the United States of America were undergoing a transformation to reduce the discrimination and segregation imposed on African Americans and the civil rights activist Martin Luther King Jr made a speech to call for an end to racism in 1963, Jane Wright became the first African American to hold such a high position at a nationally recognised institution and the first woman to be elected President of the New York Cancer Society. US President Lyndon B Johnson appointed Jane to the President's Commission of Heart Disease, Cancer and Stroke (serving 1964-1965) and the National Cancer Advisory Board (serving 1966-1970). Jane retired in 1987 by which time she had published more than 75 scientific papers, led delegations of oncologists in China, the former Soviet Union, Africa and Europe and held key positions in various international and national organisations. Jane Wright passed away on 19 February 2013 aged 93 but her legacy lives on in the name of an award from the American Association of Cancer Research.

    Background References to neurologic disorders are frequently found in fictional literature and may precede description in the medical literature. Aim Our aim was to compare Charlotte Brontë’s depiction of Bertha Mason in Jane Eyre to the tenets set forth in George Huntington’s original essay “On chorea” with the hypothesis that Mason was displaying features of Huntington disease. Results Charlotte Brontë’s 1847 Victorian novel Jane Eyre features the character Bertha Mason, who is portrayed with a progressive psychiatric illness, violent movements, and possible cognitive decline. Similar to Huntington’s tenets, Mason has a disorder with a strong family history suggestive of autosomal dominant inheritance with onset in adulthood, and culminating in suicide. Conclusion Brontë’s character had features of Huntington disease as originally described by Huntington. Brontë’s keen characterization may have increased awareness of treatment of neuropsychiatric patients in the Victorian era. PMID:26273542

  20. Optimizing Frozen Sample Preparation for Laser Microdissection: Assessment of CryoJane Tape-Transfer System®.

    Laser microdissection is an invaluable tool in medical research that facilitates collecting specific cell populations for molecular analysis. Diversity of research targets (e.g., cancerous and precancerous lesions in clinical and animal research, cell pellets, rodent embryos, etc.) and varied scientific objectives, however, present challenges toward establishing standard laser microdissection protocols. Sample preparation is crucial for quality RNA, DNA and protein retrieval, where it often determines the feasibility of a laser microdissection project. The majority of microdissection studies in clinical and animal model research are conducted on frozen tissues containing native nucleic acids, unmodified by fixation. However, the variable morphological quality of frozen sections from tissues containing fat, collagen or delicate cell structures can limit or prevent successful harvest of the desired cell population via laser dissection. The CryoJane Tape-Transfer System®, a commercial device that improves cryosectioning outcomes on glass slides has been reported superior for slide preparation and isolation of high quality osteocyte RNA (frozen bone) during laser dissection. Considering the reported advantages of CryoJane for laser dissection on glass slides, we asked whether the system could also work with the plastic membrane slides used by UV laser based microdissection instruments, as these are better suited for collection of larger target areas. In an attempt to optimize laser microdissection slide preparation for tissues of different RNA stability and cryosectioning difficulty, we evaluated the CryoJane system for use with both glass (laser capture microdissection) and membrane (laser cutting microdissection) slides.     Pearl Jane Manson enrolled in the Ontario Medical College for Women in 1904 and graduated with an M.D. degree from the University of Toronto in 1907. She was admitted to the Royal College of Surgeons of England in 1911 and worked at a hospital in England during World War I. On returning to Toronto she held positions both at Toronto General Hospital and Women's College Hospital, where she became Chief of the ENT Department in 1926. She was keenly interested in epidemiology and the prevention of infections in women and children. In 1931 she became a Fellow of the Royal College of Physicians and Surgeons of Canada and also the American College of Surgeons. She was the first woman physician in Canada to become a specialist and gain recognition as a clinical scientist.

    Sometimes the opinion regarding the cause of death in "John Doe or Jane Doe" i.e. on unknown dead bodies is a test of ability of the forensic expert and on many occasions it yields little or no results. Here the identification of the body as such poses problems; rest aside the opinion regarding the cause/ manner of death. The present 5yr study was undertaken in the Department of Forensic Medicine & Toxicology, Government Medical College & Hospital, Chandigarh to find the patterns of cause of death in unknown dead bodies, as very little literature is available with regard to John Doe or Jane Doe cases as a group, in India. Unidentified bodies comprised 4 % of the total 3165 cases brought for post-mortem examination to the department. Maximum cases belonged to the age group 41 - 50 years, 30 %. Majority of the opinions regarding the cause of death were given as "no definite opinion" (31%), followed by "cranio-cerebral damage" (30 %) and coronary insufficiency/ Cardiac disease/ aortic aneurysm rupture, (8.9%). Following measures should be undertaken to increase the chances of getting these unknown bodies identified and thereby increasing the chances of arriving at a definite cause of death: drafting of additional legislation for the management of unidentified dead bodies along with streamlining of work on the part of police, use of active investigation and modern investigative techniques, fixing the accountability of the police. Internet based sites of the police like ZIPNET (Zonal Integrated Police Networking) in Northern India, should also be used. PMID:25302219

  8. Above-normal Atlantic basin hurricane season forecast

    Between three and six major hurricanes with winds of 111 miles per hour and greater could whip across the Atlantic basin during what is forecast to be an above-normal 2011 hurricane season, according to the U.S. National Oceanic and Atmospheric Administration's Climate Prediction Center (NOAA CPC). Including those, there could be a total of 6-10 hurricanes with winds of 74 miles per hour or greater and 12-18 named storms with winds of 39 miles per hour or greater during the hurricane season, which officially begins on 1 June and lasts for 6 months, NOAA administrator Jane Lubchenco said at a 19 May briefing. There is a 70% likelihood for these ranges occurring, according to NOAA.

    The U.S. National Oceanic and Atmospheric Administration (NOAA) announced on 19 February that it will develop a Programmatic Environmental Impact Statement (PEIS) as the next step in its assessment of impacts from the Deepwater Horizon oil spill on the Gulf of Mexico. The PEIS is part of the Natural Resource Damage Assessment (NRDA), the legal process to determine the type and extent of environmental restoration needed to compensate the public for the harm caused to natural resources by the oil spill. The restoration would be paid for by BP and other parties deemed responsible for the oil spill. Public meetings will be held in the affected states to begin the PEIS process. NOAA administrator Jane Lubchenco stated at a press conference at the annual meeting of the American Association for the Advancement of Science (AAAS) that this “heralds the beginning of a formal public process.”

While the "invention of odd complaints" is characteristically a token of folly or weakness throughout her writing, in this last work imagined illness is also both a symbol and a cause of how selves and societies degenerate. In the shifting world of Sanditon, hypochondria is the lubricant for a society bent on turning health into a commodity. As a result, people's rationality and their moral character come under attack. Catherine Belling's recent subtle study, A Condition of Doubt: The Meanings of Hypochondria, unveils hypochondria's discursive and cultural character. Running sharply against the tenor of Austen's treatment, however, she argues in defense of the rationality of hypochondriacs; the notion that the condition may involve morally significant defects is not entertained; any connection to the commercialization of health care is muted. Here, I contrast Austen's morally and epistemically negative rendering of her hypochondriacal characters in Sanditon with Belling's efforts to create a sympathetic understanding of people with hypochondria. I will argue that, despite time gaps and genre differences, joint consideration of these texts can help bioethicists better appreciate how medicine can intensify, pathologize, and exploit anxieties about illness and death, thus adding to the challenges of living well in the face of mortality and morbidity.     In spite of Jane Austen's professed “eye” for an adulteress, comparatively little attention has been paid to adultery and divorce as themes and contexts of her fiction. Her unpublished epistolary novel Lady Susan has a distinctive status in Austen's oeuvre, recognized as being exemplary of her “style” and yet atypical of her later achievement. A neglected context for the novel is the extensive reporting of adultery trials in contemporary print culture and the moral panic concerning adultery in the 1780s and 1790s, focusing initially on the adulteress as the brazen woman of fashion and later as a figure of sentimentalized abjection. A particularly notorious case, that involving Lady Henrietta Grosvenor and George III's brother, the Duke of Cumberland, is directly alluded to in Lady Susan. The textual strategies of adultery trial literature, particularly its emphasis on indirection through the use of detail or “hint”, had a long-term influence on the development of Austen's fiction and her positioning of herself as a professional writer after the 1790s.

  13. Characteristics and stabilities of residues from the Wheal Jane constructed wetlands.

    The characteristics and solubilities of residues formed during effluent treatment at the Wheal Jane constructed wetland facility in Cornwall, UK are described. The constructed wetland treats an acidic (pH 3 to 4) mine discharge enriched in iron (<100 mg/l), zinc (<80 mg/l), manganese (<20 mg/l) and arsenic (<2 mg/l). The processing strategy adopted at the site is to remove elements selectively from the mine water in distinct unit process operations and concentrate the iron and base metals into residues within the circuits. Solubility tests conducted on the materials from the aerobic and anaerobic cells attempt to simulate future possible leaching processes within landfill sites used for ultimate disposal. The tests used in the evaluation included TCLP, MARG, washing and column leaching. The aerobic solids overall have very low solubilities which are well below the TCLP thresholds for As, Cd and Pb. The TCLP results for these toxic elements are supported by the washing and column test work. The anaerobic substrates are carbonaceous in nature, composed of straw, wood pulp and manure. They contain minimal amounts of toxic compounds and are considered to be less of a disposal problem. Ochre precipitated at a pH of 3-4 in the aerobic cells contains significant concentrations of arsenic (>0.1%). This element is carcinogenic, and such residues are classified as hazardous waste under current UK landfill regulations. This classification does not take into consideration the solubility of the material but is based solely on the bulk chemical composition. PMID:15680630

    Jane Addams was a social thinker, a public philosopher, and a leader of the settlement house movement in the United States in the late nineteenth and early twentieth centuries. She developed a method to understand people from backgrounds radically different from her own. This approach, which she called "sympathetic understanding," involved a dialogic process that included "perplexity" and inquiry. This process resulted in practical actions that resonated with people in the neighborhood surrounding Hull House, the settlement house she founded in Chicago. It also transformed Addams's own feeling and thinking. The process is illustrated by the "Devil Baby" tales described in Addams's work. The relationship of her method to empathy with diverse populations and professional empathy in general is discussed.

    The Jane Dent Home was established in 1898 (as the Home for Aged and Infirm Colored People) to serve African American elderly barred from admission to most homes for the aged. Sustained by community leadership through difficult times, the Home finally closed in 1975 after growing and persistent racial and economic segregation of Chicago's low-income neighborhoods combined with pressure from state government to ensure fire safety. This history illustrates the decline of not-for-profit homes for the aged while for-profit nursing homes were capturing market share. In Chicago this trend is strongest in low-income communities of color, which may lead to lower quality of care for such communities. Support for indigenous not-for-profit long-term care may promote the goals of health care equity articulated by Healthy People 2010.

    The passive treatment system designed to treat the mine water discharge of the abandoned Wheal Jane tin mine in Cornwall consisted of a sequence of artificial wetland cells, an anaerobic cell and a final series of rock filters. Three systems were operated which differed only in the pre-treatment of the mine water before discharge to the aerobic wetland cells. The aerobic cells were designed to promote aerobic oxidation and precipitation of iron which could exceed a concentration of 100 mg/l in the raw mine water discharge. The largest investment of land area was to the artificial wetland cells and it was important to understand the processes of oxidation and precipitation of iron so that the performance of this aspect the pilot passive treatment plant (PPTP) could be managed as efficiently as possible. The generally low pH of the influent mine water and inevitable trend of decreasing pH due to hydrolysis of Fe(III) meant that distinguishing between biotic and abiotic mechanisms was fundamental for further design planning of passive treatment systems.   20. Spatial distribution of iron oxidation in the aerobic cells of the Wheal Jane Pilot Passive Treatment Plant.

    The wetland cells of the Wheal Jane Pilot Passive Treatment Plant (PPTP) were designed to promote aerobic oxidation and precipitation of iron which could exceed a concentration of 100 mg l-1 in the raw mine water. The largest investment of land area was to the wetland (also called aerobic) cells and it was important to understand the processes of oxidation and precipitation of iron so that the performance of this part of the pilot passive treatment plant (PPTP) could be managed efficiently. The results of a high-resolution sampling programme on the distribution of Fe(II) within the first wetland cell of each treatment system are described. Comparison of inflow and outflow concentrations of iron adequately described the performance of the lime-dosed (LD) system. However, precipitation of iron in the anoxic limestone drain (ALD) and lime-free systems (LFS) was more efficient. On average, about 90% of the iron present in the inflow was removed using only 50% and 33% of the first aerobic cells of the ALD and LFS systems, respectively. As the concentration of iron approached 20 mg l-1, the rate of oxidation slowed considerably. This was probably due to be due to low pH levels caused by hydrolysis of Fe(III). With the introduction of passive pH control mechanisms, there was capacity to increase the volume of mine water treated by the ALD and LDS systems by 10 and 15 times, respectively, but it is uncertain as to whether or not other aspects of the passive treatment system would have sufficient capacity to deal with the increased volumes of mine water.     Jane Austen is typically described as having excellent health until the age of 40 and the onset of a mysterious and fatal illness, initially identified by Sir Zachary Cope in 1964 as Addison's disease. Her biographers, deceived both by Cassandra Austen's destruction of letters containing medical detail, and the cheerful high spirits of the existing letters, have seriously underestimated the extent to which illness affected Austen's life. A medical history reveals that she was particularly susceptible to infection, and suffered unusually severe infective illnesses, as well as a chronic conjunctivitis that impeded her ability to write. There is evidence that Austen was already suffering from an immune deficiency and fatal lymphoma in January 1813, when her second and most popular novel, Pride and Prejudice, was published. Four more novels would follow, written or revised in the shadow of her increasing illness and debility. Whilst it is impossible now to conclusively establish the cause of her death, the existing medical evidence tends to exclude Addison's disease, and suggests there is a high possibility that Jane Austen's fatal illness was Hodgkin's disease, a form of lymphoma.

    The 2009 annual update of the Arctic Report Card, issued on 22 October, indicates that “warming of the Arctic continues to be widespread, and in some cases dramatic. Linkages between air, land, sea, and biology are evident.” The report, a collaborative effort of 71 national and international scientists initiated in 2006 by the Climate Program Office of the U.S. National Oceanic and Atmospheric Administration (NOAA), highlights several concerns, including a change in large-scale wind patterns affected by the loss of summer sea ice; the replacement of multiyear sea ice by first-year sea ice; warmer and fresher water in the upper ocean linked to new ice-free areas; and the effects of the loss of sea ice on Arctic plant, animal, and fish species. “Climate change is happening faster in the Arctic than any other place on Earth-and with wide-ranging consequences,” said NOAA administrator Jane Lubchenco. “This year“s Arctic Report Card underscores the urgency of reducing greenhouse gas pollution and adapting to climate changes already under way.”

    The White House's proposed fiscal year (FY) 2013 budget for the National Oceanic and Atmospheric Administration (NOAA), announced on 13 February, looks favorable at first glance. The administration's request calls for $5.1 billion, an increase of $153 million (3.1%) above the FY 2012 estimated budget. However, the increase for NOAA satellites is $163 million, which means that other areas within the agency would be slated for decreased funding, including programs within the National Ocean Service (NOS), National Marine Fisheries Service (NMFS), National Weather Service (NWS), and some NOAA education programs. The proposed overall budget for the agency “reflects the overarching importance of weather satellites to public safety, to national security, and to the economy,” NOAA director Jane Lubchenco said at a 16 February briefing, noting that difficult choices were made regarding the budget. “Due to significant resources required for our weather satellites and the economic conditions in the country, other parts of our budget have been reduced, in some cases quite significantly,” she said. She added that the imperative to fund both the Joint Polar Satellite System (JPSS) and geostationary satellites in FY 2013 “imposes serious constraints on the rest of NOAA's budget.”

    The Marine Biological Laboratory (MBL) in Woods Hole, MA provided opportunities for women to conduct research in the late 19th and early 20th century at a time when many barriers existed to their pursuit of a scientific career. One woman who benefited from the welcoming environment at the MBL was Mary Jane Hogue. Her remarkable career as an experimental biologist spanned over 55 years. Hogue was born into a Quaker family in 1883 and received her undergraduate degree from Goucher College. She went to Germany to obtain an advanced degree, and her research at the University of Würzburg with Theodor Boveri resulted in her Ph.D. (1909). Although her research interests included experimental embryology, and the use of tissue culture to study a variety of cell types, she is considered foremost a protozoologist. Her extraordinary demonstration of chromidia (multiple fission) in the life history of a new species of Flabellula associated with diseased oyster beds is as important as it is ignored. We discuss Hogue's career path and her science to highlight the importance of an informal network of teachers, research advisors, and other women scientists at the MBL all of whom contributed to her success as a woman scientist.

    The basic systems of any society rarely can operate independently. Instead, they are dependent and often interdependent upon other entities. Such entities control the resources within their respective systems. Thus, coordination and control agencies require contracts or memoranda of understanding with these entities in order to assure access to the resources required during a crisis. These administrative structures include: (1) governmental institutions and agencies, including the military; (2) intergovernmental organisations; (3) nongovernmental organisations; (4) commercial private sector organisations; and (5) academic institutions. These dependencies create potential barriers to the provision of coordination and control including: (1) the complexity of the administrative structures with which coordination and control must interact; (2) the location of resources; (3) finding responsible person(s); (4) the competence and compatibility; (5) methods of access; (6) payment; (7) contracts and memoranda of understanding; (8) inventories of accessible resources; (9) competition for the mandate, power, and resources; and (10) jealousy. The need for potential interactions between administrative structures requires that agreements for the sharing of resources during crises be reached as part of planning and preparedness. Gaining an understanding of these relationships is an important area for research.

    The Acting Public Advocate of Victoria, Australia, sought an injunction restraining the parents of a mentally retarded 17-year-old girl with the mental age of two years from having her sterilized without the consent of the Court. The evidence showed that the girl would have great difficulty dealing with menstruation and pregnancy and would not be able to cope with motherhood. In consenting to a hysterectomy, the Court held that the question was whether the girl's welfare required the operation and that, in the case of such a serious operation, the degree of satisfaction required must be more than a mere tipping of the balance in its favor. The Court determined that in such cases the incompetent person should have separate legal representation and the judge must find that the incompetent person lacks and will continue to lack the capacity to make a decision about sterilization. In determining whether sterilization is in the individual's best interest, the Court should consider 1) whether the incompetent person can become pregnant; 2) whether the incompetent person would experience trauma or psychological damage from pregnancy or childbirth or, conversely, from the sterilization; 3) the likelihood of voluntary sexual activity or rape; 4) the inability of the incompetent person to understand reproduction or contraception at the present time or in the future; 5) the feasibility of less drastic means of contraception; 6) the advisability of sterilization at the time of the application rather than in the future; 7) the ability of the incompetent person to care for a child; 8) evidence that medical or scientific advances may result in improvement of the person's condition or less drastic sterilization procedures; and 9) whether the proponents of sterilization are seeking it in good faith and in the best interests of the incompetent person rather than for their own or the public's convenience. The Court also ruled 1) that the girl's right to bodily inviolability and to reproduce should be considered but should not override the concern for her welfare; 2) international human rights declarations acceded to by Australia were not expressly incorporated into domestic law and, thus, did not need to be applied to this case; and 3) that a court must consent before any medical procedure which interferes with human rights or is nontherapeutic is performed on a child or incompetent person. We are clarifying that FDA may require the IRB to withhold approval of new FDA-regulated studies, stop the enrollment of new subjects in ongoing studies, and terminate ongoing studies, or any combination of these actions until the noncompliance with FDA's IRB regulations is corrected. We are taking this action to ensure clarity and improve the accuracy of the regulations.

    We describe a case of accidental intrathecal administration of vincristine in a 33-year-old man with clinical diagnosis of acute lymphocytic leukemia. The patient died 20 days after receiving the drug. Clinically, the patient developed acute ascending paralysis with motor and sensory dysfunctions, and respiratory failure. Neuropathological investigation revealed lesions in spinal cord, roots, and cerebellum characterized by rarefaction of the neuropil, axonal, and myelin degeneration, accompanied by macrophagic infiltration.

    NASA has requested a $14.3 billion budget for fiscal year 1995, which is $250 million below the $14.5 billion it received in fiscal 1994. NASA Administrator Daniel S. Goldin said the budget conveys the president's commitment to a strong and vital aeronautics and space program while reflecting today's very real fiscal constraints. No major programs were canceled.“The president's budget plan for NASA is a sound one, with reductions that still preserve a meaningful space program, but this is it. We can't get any closer to the bone,” Goldin warned. In real terms, this 5-year fiscal 1995 budget represents a 30% cut in the last 2 years. “We must hold the line at this level and will work closely with Congress to do so,” Goldin said.

    Time is an integral part of the Bonneville Power Administration's (BPA) operational systems. Generation and power transfers are planned in advance. Utilities coordinate with each other by making these adjustments on a timed schedule. Price varies with demand, so billing is based on time. Outages for maintenance are scheduled to assure they do not interrupt reliable power delivery. Disturbance records are aligned with recorded timetags for analysis and comparison with related information. Advanced applications like traveling wave fault location and real-time phase measurement require continuous timing with high precision. Most of BPA is served by a Central Time System (CTS) at the Dittmer Control Center near Portland, OR. This system keeps time locally and supplies time to both the control center systems and field locations via a microwave signal. It is kept synchronized to national standard time and coordinated with interconnected utilities. It is the official BPA time. Powwer system control and operation is described, followed by a description of BPA timing systems including CTS, the Fault Location Acquisition Reporter, time dissemination, and phasor measurements.     A three semester, 60-credit course package in the topic of Administrative Data Processing (ADP), offered in 1966 at Stockholm University (SU) and the Royal Institute of Technology (KTH) is described. The package had an information systems engineering orientation. The first semester focused on datalogical topics, while the second semester focused on the infological topics. The third semester aimed to deepen the students’ knowledge in different parts of ADP and at writing a bachelor thesis. The concluding section of this paper discusses various aspects of the department’s first course effort. The course package led to a concretisation of our discipline and gave our discipline an identity. Our education seemed modern, “just in time”, and well adapted to practical needs. The course package formed the first concrete activity of a group of young teachers and researchers. In a forty-year perspective, these people have further developed the department and the topic to an internationally well-reputed body of knowledge and research. The department has produced more than thirty professors and more than one hundred doctoral degrees.

    Spinal anaesthetics can induce histopathologic lesions and regional haemodynamic alterations in the spinal cord. There are numerous causes of neurologic lesions, including direct trauma of the spinal cord and nerve roots during puncture or catheter insertion, compromised spinal cord perfusion and direct neurotoxic effect. Histopathologic lesions are localized either in meninges (meningitis or arachnoiditis) or in neuraxis (myelitis or axonal degeneration). Neurotoxicity can result from decrease in neuronal blood supply, elicited by high concentrations of the solutions, long duration exposure to local anaesthetics, and the use of adjuvants. They have been implicated in the occurrence of cauda equina syndrome after continuous spinal anaesthesia using hyperbaric solution of lidocaine and tetracaine given through small diameter catheters. Selective spinal analgesia is induced by spinal opioids without motor blockade except for meperidine. Complications occurred in patients after high doses of morphine, which were related to one of its metabolites, morphine-3-glucuronide. Preservative-free opioid solutions are to be preferred for spinal anaesthesia. There is no report of neurotoxicity neither in animal studies, nor in humans, using spinal clonidine. In order to reduce the incidence of neurotoxicity, some safety rules should be followed. The lowest efficient dose of local anaesthetics must be given. Incomplete blockade should not necessarily lead to a reinjection. Large volume of hyperbaric lidocaine or repeated injections of such solutions must be avoided as well as preservative-containing solutions. The administration of new compounds by the spinal route must be supported by data of spinal neuropharmacology and the lack of neurotoxicity must have been previously checked with animal studies.

  15. Impact of an electronic medication administration record on medication administration efficiency and errors.

    The study aims were to evaluate the impact of electronic medication administration record implementation on medication administration efficiency and occurrence of medication errors as well as to identify the predictors of medication administration efficiency in an acute care setting. A prospective, observational study utilizing time-and-motion technique was conducted before and after electronic medication administration record implementation in November 2011. A total of 156 cases of medication administration activities (78 pre- and 78 post-electronic medication administration record) involving 38 nurses were observed at the point of care. A separate retrospective review of the hospital Midas+ medication error database was also performed to collect the rates and origin of medication errors for 6 months before and after electronic medication administration record implementation. The mean medication administration time actually increased from 11.3 to 14.4 minutes post-electronic medication administration record (P = .039). In a multivariate analysis, electronic medication administration record was not a predictor of medication administration time, but the distractions/interruptions during medication administration process were significant predictors. The mean hospital-wide medication errors significantly decreased from 11.0 to 5.3 events per month post-electronic medication administration record (P = .034). Although no improvement in medication administration efficiency was observed, electronic medication administration record improved the quality of care with a significant decrease in medication errors.
