    With the increasing nursing shortage, it is imperative that nursing programs attract and retain students who will be successful in becoming competent, caring practitioners. High school guidance counselors can influence the career choices of students. To do so, these professionals must have an accurate understanding of the attributes that will facilitate a career in nursing, as well as the changing roles of nurses. This study determined the current perceptions of high school guidance counselors regarding nursing. Overall, guidance counselors had realistic perceptions of nursing and identified personal and cognitive attributes of students that would promote success in nursing. However, nurses involved in the recruitment of students need to ensure that guidance counselors have more accurate perceptions of the changing role of nurses, opportunities for practice outside hospital settings, and the importance of problem-solving abilities and leadership skills for nurses.   9. School counselors as gatekeepers: guidance in poor versus affluent neighborhoods.

    Previous studies reveal that career guidance is usually based on a mixture of meritocratic criteria (academic ability and achievement) and nonmeritocratic considerations (students' SES and their social labeling by counselors). The present study argues that school counselors in affluent neighborhoods emphasize the meritocratic criteria, while those in poorer neighborhoods tend to focus on the more nonmeritocratic. This hypothesis is examined in a random sample of 210 male, primary school graduates from the 10 state schools of an Israeli town, 125 of whom are from schools in affluent neighborhoods, and 85 from schools in poorer neighborhoods. A path analysis of determinants of guidance to specific high school tracks shows that students' ethnicity and labeling as problematic directly affect guidance by counselors only in the poor neighborhoods. The effect of students' "problems" is due to their being labeled as problematic with respect to psychological or physical attributes rather than to behavioral problems at school. The findings are subsequently interpreted as reflecting a patronizing attitude toward lower status students by counselors. Additional studies which may further elaborate the determinants of guidance in poor versus affluent neighborhoods are proposed. PMID:3434387

  15. Connecting with guidance counselors to enhance recruitment into nursing of minority teens.

    The purpose of this article was to describe the design, implementation, and outcomes of one strategy used to promote careers in nursing among minority high-school-aged teens-namely, a workshop for guidance counselors in a large urban school system. All guidance counselors in an urban inner city school system were invited to attend a workshop on careers in nursing as part of their monthly continuing education requirements. Thirty-nine (39%) participated in the half-day workshop held at a school of nursing. Most informative were the guidance counselor comments about perceptions of nursing careers that create barriers to recruitment. Guidance counselors reported that (a) many students and parents view nursing as a "dangerous field," (b) negative stereotypes about nursing persist, (c) many students do not plan for the prerequisites for baccalaureate entrance, (d) nursing is perceived as a "narrow" field with few opportunities for advancement, and (e) there is a preference of 2-year programs to get "quick income" over baccalaureate programs and long-term career planning. Faculty and students addressed these issues. Faculty collaboration with guidance counselors is an excellent mechanism to uncover barriers to minority recruitment at the local level. Nursing recruitment activities should be tailored to address these community concerns. In the last 4 years, 24 students from the targeted high school system were offered admission to the University at Buffalo and of this group 17 enrolled in nursing. PMID:19022212

Guidance on the safe use of MABs is mainly to be found in national documents covering all systemic anti-cancer therapies, with the only consensus guidelines specifically on MABs coming from Australia.     The purpose of this memorandum and its attachments is to provide guidance on administrative response cost settlements entered under Section 122(h)(1) of the Comprehensive Environmental Response, Compensation, and Liability Act, and on administrative ``cashout`` settlements with peripheral parties under Section 122(h)(1) of CERLCA and the authority of the Attorney General. Part 1 of this memorandum provides an overview of the guidance and explains the context in which the attached model settlements should be used. Part 2 of this memorandum explains the statutory provision, defines relevant terms, discusses the various types of administrative response cost settlements and the scope of covenants not to sue and reservations of rights in those settlements, provides guidance on amount of payment and use of premiums in future cost settlements, provides guidance on amount of payment and use of premiums in future cost settlements, and briefly discusses contribution protection clauses in such settlements.     The February 2013 Food and Drug Administration (FDA) draft guidance for developing drugs for early-stage Alzheimer's disease (AD) creates certain challenges as they guide toward the use of one cognitive outcome to gain accelerated marketing approval for preclinical AD drugs, and a composite clinical scale - the Clinical Dementia Rating Scale in particular - for the primary outcome for prodromal AD clinical trials. In light of the developing knowledge regarding early stage diagnoses and clinical trials outcomes, we recommend that FDA describe its requirements for validating preclinical AD diagnoses for drug development purposes, maintain the principle for requiring coprimary outcomes, and encourage the advancement of outcomes for early stage AD trials. The principles for drug development for early stage AD should not differ from those for clinical AD, especially as the diagnoses of prodromal and early AD impinge on each other. The FDA should not recommend that a composite scale be used as a sole primary efficacy outcome to support a marketing claim unless it requires that the cognitive and functional components of such a scale are demonstrated to be individually meaningful. The current draft guidelines may inadvertently constrain efforts to better assess the clinical effects of new drugs and inhibit innovation in an area where evidence-based clinical research practices are still evolving.     The ICH Q1B guidance and additional clarifying manuscripts provide the essential information needed to conduct photostability testing for pharmaceutical drug products in the context of manufacturing, packaging, and storage. As the previous 2 papers in this series highlight for drug products administered by injection (part 1) and drug products administered via topical application (part 2), there remains a paucity of guidance and methodological approaches to conducting photostability testing to ensure effective product administration. Part 3 in the series is presented here to provide a similar approach and commentary for photostability testing for oral drug products. The approach taken, as was done previously, is to examine "worst case" photoexposure scenarios in combination with ICH-defined light sources to derive a set of practical experimental approaches to support the safe and effective administration of photosensitive oral drug products.     Understanding factors associated with incentive-related human resource practices for substance use disorder counselors can help promote a stable workforce in this occupation. We examined three counselor incentives—salaries, benefits, training—and the link with organizational, counselor, and patient characteristics. Data were collected in 2007/08 via face-to-face interviews with 345 administrators/clinical directors in private treatment centers. Centers paid counselors an average of $38,800 annually and provided a mean of 2.83 benefits and 1.61 training (0-4 scales). Characteristics differed based on the incentive. Centers’ managements need to be aware of different incentives that can help attract and retain counselors. PMID:22039315

    Background Evidence supports the implementation of task shifting to address health worker shortages that are common in resource-limited settings. However, there is need to learn from established programs to identify ways to achieve the strongest, most sustainable impact. This study examined the Botswana lay counselor cadre, a task shifting initiative, to explore effectiveness and contribution to the health workforce. Methods This evaluation used multiple methods, including a desk review, a national lay counselor survey (n = 385; response = 94%), in-depth interviews (n = 79), lay counselors focus group discussions (n = 7), lay counselors observations (n = 25), and client exit interviews (n = 47). Results Interview and focus group data indicate that lay counselors contribute to essentially all HIV-related programs in Botswana and they conduct the majority of HIV tests and related counseling at public health facilities throughout the country. Interviews showed that the lay counselor cadre is making the workload of more skilled health workers more manageable and increasing HIV acceptance in communities. The average score on a work-related knowledge test was 74.5%. However for 3 questions, less than half answered correctly. During observations, lay counselors demonstrated average competence for most skills assessed and clients (97.9%) were satisfied with services received. From the survey, lay counselors generally reported being comfortable with their duties; however, some reported clinical duties that extended beyond their training and mandate. Multiple factors affecting the performance of the lay counselors were identified, including insufficient resources, such as private counseling space and HIV test kits; and technical, administrative, and supervisory support. Conclusion Lay counselors are fulfilling an important role in Botswana's healthcare system, serving as the entry point into HIV care, support, and treatment services. Recommendation

    The Supreme Court decision of January 22, 1973, legalizing abortion now requires school counselors to examine both their personal and professional positions on abortion information and abortion counseling. To date a review of school counseling literature reveals a failure to deal with abortion as a counseling issue. Also, schools have failed to develop official policies regarding abortion counseling and the distribution of abortion information. The counselors who have provided abortion information to date have done so at the request of a student or parent rather than by making the information generally available. A study in 1973 in Virginia, however, revealed that Virginia counselor educators believed that there was a need for counselors in training to be exposed to abortion information as part of their formal training experience. Generally, today's present exposure to abortion information makes it impossible for counselors to continue to ignore a growing demand for both abortion information and counseling. School counselors must deal with the following questions: 1) What course of action should school counselors take when a pregnant young seeks counseling on alternatives to pregnancy continuation? 2) What is the counselor's professional role in abortion counseling with respect to his or her personal feelings and beliefs? 3) What kind of training if any should school counselors receive regarding abortion counseling? 4) Is there a need for in-service training on abortion counseling for school counselors? 5) Should various professional organizations develop materials that would assist their members in providing abortion counseling? 6) Should institutions such as schools, churches, and community agencies establish policies concerning abortion counseling?       Sickle cell disease (SCD), sickle cell trait (SCT) and related conditions are highly prevalent in sub-Saharan Africa. Despite the public health implications, there is limited understanding of the unique needs regarding establishing and implementing extensive screening for newborns and appropriate family counseling. We sought to gain understanding of community attitudes and beliefs about SCD/SCT from counselors and potential counselors in Ghana; obtain their input about goals for counseling following newborn screening; and obtain guidance about developing effective counselor education. Five focus groups with 32 health care providers and health educators from 9 of 10 regions in Ghana were conducted by trained facilitators according to a structured protocol. Qualitative data were coded and categorized to reflect common themes. Saturation was achieved in themes related to genetics/inheritance; common complications of SCD; potential for stigmatization; marital strain; and emotional stress. Misconceptions about SCT as a form of SCD were prevalent as were cultural and spiritual beliefs about the causes of SCD/SCT. Potential positive aspects included affected children's academic achievement as compensation for physical limitations, and family cohesion. This data informed recommendations for content and structure of a counselor training program that was provided to the Ministry of Health in Ghana. PMID:25193810

      The U.S. Food and Drug Administration (FDA) is warning against the use of laparoscopic power morcellators in the majority of women undergoing myomectomy or hysterectomy for the treatment of leiomyomas because of the concern for inadvertent spread of tumor cells if an undiagnosed cancer were present. The authors, representing a 45-member review group, reviewed the current literature to formulate prevalence rates of leiomyosarcoma in women with presumed leiomyomas and to asses reliable data regarding patient survival after morcellation. The authors disagree with the FDA's methodology in reaching their conclusion and provide clinical recommendations for care of women with leiomyomas who are planning surgery. PMID:26646134

      This paper addresses statistical issues in non-inferiority trials where the primary outcome is a fatal event. The investigations are inspired by a recent Food and Drug Administration (FDA) draft guideline on treatments for nosocomial pneumonia. The non-inferiority margin suggested in this guideline for the endpoint all-cause mortality is defined on different distance measures (rate difference and odds ratio) and is discontinuous. Furthermore, the margin enables considerable power for the statistical proof of non-inferiority at alternatives that might be regarded as clinically unacceptable, that is, even if the experimental treatment is harmful as compared with the control. We investigated the appropriateness of the proposed non-inferiority margin as well as the performance of possible test statistics to be used for the analysis. A continuous variant of the margin proposed in the FDA guideline together with the unconditional exact test according to Barnard showed favorable characteristics with respect to type I error rate control and power. To prevent harmful new treatments from being declared as non-inferior, we propose to add a 'second hurdle'. We discuss examples and explore power characteristics when requiring both statistical significance and overcoming the second hurdle. PMID:22991269

      Genetic counselors (GCs) have recently begun moving into non-clinic based roles in increasing numbers. A relatively new role for GCs is working for startup companies. Startups are newly established companies in the phase of developing and researching new scalable businesses. This article explores the experiences of four GCs working at different startup companies and aims to provide resources for GCs interested in learning more about these types of roles. The article describes startup culture, including a relatively flat organizational structure, quick product iterations, and flexibility, among other unique cultural characteristics. Financial considerations are described, including how to understand and evaluate a company's financial status, along with a brief explanation of alternate forms of compensation including stock options and equity. Specifically, the article details the uncertainties and rewards of working in a fast-paced startup environment that affords opportunities to try new roles and use the genetic counseling skill set in new ways. This article aims to aid GCs in determining whether a startup environment would be a good fit, learning how to evaluate a specific startup, and understanding how to market themselves for positions at startups.       Should counselors with interpersonal trauma histories work with similarly traumatized clients? How does the work affect them? Current research is inconsistent. This study examines 101 sexual assault and domestic violence counselors' recalled motivations for trauma work, their reported subjective personal changes, and their secondary and vicarious trauma symptoms and burnout. Counselors motivated by interpersonal trauma report both more symptoms and positive changes (including dealing with their own trauma). Those seeking personal meaning report becoming more hypervigilant and self-isolating. Those saying they learned from clients rate symptoms lower, suggesting stress inoculation. Supervisors of trauma counselors should facilitate learning from clients separately from processing the counselor's trauma. PMID:20956440

      The directive clarifies the requirement that an endangerment assessment be developed to support all administrative and judicial enforcement actions under Section 106 of the Comprehensive Environmental Response, Compensation, and Liability Act (CERCLA) and Section 7003 of the Resource Conservation and Recovery Act (RCRA). Before taking enforcement action under these provisions to abate the hazards or potential hazards at a site, the Environmental Protection Agency (EPA) must be able to properly document and justify its assertion that an imminent and substantial endangerment to public health or welfare or the environment may exist. The endangerment assessment provides this documentation and justification. The endangerment assessment is not necessary to support Section 104 actions. It also provides guidance on the content, timing, level of detail, format, and resources required for the preparation of endangerment assessments.

      Patient anger is challenging for healthcare professionals to manage, particularly when it is directed at them. This study comprises the first in-depth investigation of genetic counselors' experiences with patient anger. Using a brief survey and interview methods, this study explored prevalence and context of patient anger directed at the genetic counselor, how genetic counselors manage patient anger directed at them, and possible thematic differences due to genetic counseling experience. Individuals enrolled in the National Society of Genetic Counselors (NSGC) listserv were invited to participate in a study of their experiences with patient anger directed at them. A majority of survey respondents (95.7 %, 243/254) reported experiencing patient anger directed at them, and 19.4 % reported having feared for their safety because of patient anger. Twenty-two survey respondents were purposively selected to participate in individual interviews. Inductive and cross case analysis yielded prevalent themes concerning patient triggers for anger, including bad news, logistical mishaps, and perceived counselor characteristics. Interview results further suggest unaddressed patient anger negatively affected patient and counselor emotional well-being and hindered genetic counseling goals. Prevalent challenges included genetic counselor attempts to accurately recognize, understand, and effectively manage patient anger without taking it personally. Commonly recommended strategies for addressing anger were empathy (i.e., understanding origins of patient anger), anticipating and acknowledging anger, maintaining personal, professional and legal protection, and debriefing with colleagues. Themes were quite similar across counselor experience levels. The findings underscore the importance of training and continuing education regarding patient anger.
