Exposure of the brain to ionizing radiation might promote the development of Alzheimer's disease (AD). Analysis of AD death rates versus radon background radiation and total background radiation in U.S. states. Total background, radon background, cosmic and terrestrial background radiation measurements are from Assessment of Variations in Radiation Exposure in the United States and Report No. 160 - Ionizing Radiation Exposure of the Population of the United States. 2013 AD death rates by U.S. state are from the Alzheimer's Association. Radon background ionizing radiation was significantly correlated with AD death rate in 50 states and the District of Columbia (r = 0.467, p = 0.001). Total background ionizing radiation was also significantly correlated with AD death rate in 50 states and the District of Columbia (r = 0.452, p = 0.001). Multivariate linear regression weighted by state population demonstrated that AD death rate was significantly correlated with radon background (β= 0.169, p < 0.001), age (β= 0.231, p < 0.001), hypertension (β= 0.155, p < 0.001), and diabetes (β= 0.353, p < 0.001). Our findings, like other studies, suggest that ionizing radiation is a risk factor for AD. Intranasal inhalation of radon gas could subject the rhinencephalon and hippocampus to damaging radiation that initiates AD. The damage would accumulate over time, causing age to be a powerful risk factor.

Bioaerosols are well-known immune-active particles that exacerbate respiratory diseases. Human exposures to bioaerosols and their resultant health impacts depend on their ambient concentrations, seasonal and spatial variation, and copollutants, which are not yet widely characterized. In this study, chemical and biological tracers of bioaerosols were quantified in respirable particulate matter (PM10) collected at three urban and three background sites in the Midwestern United States across four seasons in 2012. Endotoxins from Gram-negative bacteria (and a few Gram-positive bacteria), water-soluble proteins, and tracers for fungal spores (fungal glucans, arabitol, and mannitol) were ubiquitous and showed significant seasonal variation and dependence on temperature. Fungal spores were elevated in spring and peaked in summer, following the seasonal growing cycle, while endotoxins peaked in autumn during the row crop harvesting season. Paired comparisons of bioaerosols in urban and background sites revealed significant urban enhancements in PM10, fungal glucans, endotoxins, and water-soluble proteins relative to background locations, such that urban populations have a greater outdoor exposure to bioaerosols. These bioaerosols contribute, in part, to the urban excesses in PM10. Higher bioaerosol mass fractions in urban areas relative to background sites indicate that urban areas serve as a source of bioaerosols. Similar urban enhancements in water-soluble calcium and its correlation with bioaerosol tracers point toward windblown soil as an important source of bioaerosols in urban areas.

  16. The financial burden of out-of-pocket expenses in the United States and Canada: How different is the United States?

    PubMed Central

    Baird, Katherine E

    2016-01-01

Background: This article compares the burden that medical cost-sharing requirements place on households in the United States and Canada. It estimates the probability that individuals with similar demographic features in the two countries have large medical expenses relative to income. Method: The study uses 2010 nationally representative household survey data harmonized for cross-national comparisons to identify individuals with high medical expenses relative to income. Using logistic regression, it estimates the probability of high expenses occurring among 10 different demographic groups in the two countries. Results: The results show the risk of large medical expenses in the United States is 1.5–4 times higher than it is in Canada, depending on the demographic group and spending threshold used. The United States compares least favorably when evaluating poorer citizens and when using a higher spending threshold. Conclusion: Recent health care reforms can be expected to reduce Americans’ catastrophic health expenses, but it will take very large reductions in out-of-pocket expenditures—larger than can be expected—if poorer and middle-class families are to have the financial protection from high health care costs that their counterparts in Canada have. 17. The Influence of Geography and Geology on Seismic Background Noise Levels Across the United States as Revealed by the Transportable Array

    NASA Astrophysics Data System (ADS)

The EarthScope USArray Transportable Array (TA) has now covered the US with 3-component broadband seismometers at approximately 70 km station spacing and deployment durations of approximately 2 years. This unprecedented coverage, combined with high-quality and near homogenous installation techniques, offers a novel dataset in which to characterize spatially varying levels of background seismic noise across the United States. We present background noise maps in period bands of interest to earthquake and imaging seismology across the US (lower 48 states and Alaska). Early results from the contiguous 48 states demonstrate that ambient noise levels within the body wave period band (1-5 s) vary by > 20 dB (rel. 1 (m/s2)2/Hz) with the highest noise levels occurring at stations located within sedimentary basins and lowest within the mountain ranges of the Western US. Additionally, stations around the Great Lakes observe heightened noise levels in this band beyond the aforementioned basin amplification. We attribute this observation to local swell activity in the Great Lakes generating short-period microseism signals. This suggests that lake-generated microseisms may be a significant source of noise for Alaskan deployments situated in close proximity to lakes to facilitate float plane access. We further investigate how basin amplification and short-period lake microseism signals may noticeably impact detection and signal-to-noise of teleseismic body wave signals during certain time periods. At longer-periods (> 20 s), we generally observe larger noise levels on the horizontal components of stations situated in basins or on soft sediment, likely caused by locally induced tilt of the sensor. We will present similar analysis from the initial Alaska TA dataset to quantitatively assess how utilization of posthole sensors affects signal-to-noise for the long-period horizontal wavefield.

To quantify the proportion of academic neurosurgeons practicing in the United States who acquired residency training outside of the United States and compare their training backgrounds and academic success with those who received their residency training in the United States. We identified 1338 clinically active academic neurosurgeons from 104 programs that participated in the neurosurgery residency match in the United States in January-February 2015. Their training backgrounds, current academic positions, and history of National Institutes of Health (NIH) grant awards between 2005 and 2014 were retrieved from publicly accessible sources. Eighty-four U.S. academic neurosurgeons (6.3%) received their residency training in 20 different countries outside of the United States/Puerto Rico, representing all major regions of the world. The majority trained in Canada (n = 48). We found no major differences between the foreign-trained and U.S.-trained neurosurgeons in male:female ratio, year of starting residency, proportion with positions in medical schools ranked in the top 15 by the U.S. News and World Report, general distribution of academic positions, and proportion with an NIH grant. Compared with U.S.-trained academic neurosurgeons, foreign-trained academic neurosurgeons had a significantly higher proportion of Ph.D. degrees (32.1% vs. 12.3%; P < 0.0001) and held more associate professorships (34.5% vs. 23.1%; P = 0.02). The academic practices of the foreign-trained neurosurgeons were widely distributed throughout the United States. A small group of U.S. academic neurosurgeons (6.3%) have acquired residency training outside of the United States, representing all major regions of the world. Their general demographic data and academic accomplishments are comparable to those of U.S.-trained neurosurgeons. Copyright © 2017 Elsevier Inc. All rights reserved.

  10. An exploration of the impact of family background factors on the science achievement of Afro-Caribbean and African American students in the United States

    NASA Astrophysics Data System (ADS)

    Pinder, Patrice J.

Ogbu and Simons (1998) defined voluntary immigrants as individuals who chose to migrate to the United States (U.S.). Involuntary immigrants are defined as individuals whose ancestors were brought to the U.S. by force (Obgu & Simons, 1998). There have been recent reports indicating that voluntary immigrants are outperforming involuntary immigrants (Fisher, 2005; Williams, Fleming, Jones, & Griffin, 2007). There seems to be a trend in voluntary immigrants exhibiting a higher academic achievement pattern than involuntary immigrants (Fisher, 2005; Rong & Preissle, 1998; Williams et al., 2007). However, the reason for the groups' differences in achievement has not been extensively explored. The primary objective of this research study was to explore the impact of family background on the academic achievement patterns of Afro-Caribbean and African American students in the United States. The study utilized two research designs; a causal-comparative and a correlational design. A questionnaire was distributed to a sample of eighty-seven high school students. Eighteen of the participants were Afro-Caribbean students, and sixty-seven were African American students. Chemistry test scores for the students were also provided. The results of the study indicated that Afro-Caribbean students outperformed African American students on the test of science achievement. The difference was statistically significant (t= 2.43, p<0.05). Additionally, results suggested that there were a few significant differences in Afro-Caribbean and African American students' family backgrounds. Moreover, the findings of this study suggest that the positive impact of arrival status on the first-generation of Afro-Caribbean immigrants may be influencing their children's academic success in science. The present study holds a few implications for parents and teachers of immigrant minority students. Additionally, the current researcher has offered several implications for future research on ethnicity

  6. Cost Effectiveness of Contraceptives in the United States

    PubMed Central

Trussell, James; Lalla, Anjana M.; Doan, Quan V.; Reyes, Eileen; Pinto, Lionel; Gricar, Joseph

    2013-01-01

Background The study was conducted to estimate the relative cost effectiveness of contraceptives in the United States from a payer’s perspective. Methods A Markov model was constructed to simulate costs for 16 contraceptive methods and no method over a 5-year period. Failure rates, adverse event rates, and resource utilization were derived from the literature. Sensitivity analyses were performed on costs and failure rates. Results Any contraceptive method is superior to “no method”. The three least expensive methods were the copper-T IUD ($647), vasectomy ($713) and LNG-20 IUS ($930). Results were sensitive to the cost of contraceptive methods, the cost of an unintended pregnancy, and plan disenrollment rates. Conclusion The copper-T IUD, vasectomy, and the LNG-20 IUS are the most cost-effective contraceptive methods available in the United States. Differences in method costs, the cost of an unintended pregnancy, and time horizon are influential factors that determine the overall value of a contraceptive method. PMID:19041435

  3. Investigation of background radiation levels and geologic unit profiles in Durango, Colorado

DOE Office of Scientific and Technical Information (OSTI.GOV)

    Triplett, G.H.; Foutz, W.L.; Lesperance, L.R.

    1989-11-01

As part of the Uranium Mill Tailings Remedial Action (UMTRA) Project, Oak Ridge National Laboratory (ORNL) has performed radiological surveys on 435 vicinity properties (VPs) in the Durango area. This study was undertaken to establish the background radiation levels and geologic unit profiles in the Durango VP area. During the months of May through June, 1986, extensive radiometric measurements and surface soil samples were collected in the Durango VP area by personnel from ORNL's Grand Junction Office. A majority of the Durango VP surveys were conducted at sites underlain by Quaternary alluvium, older Quaternary gravels, and Cretaceous Lewis and Mancosmore » shales. These four geologic units were selected to be evaluated. The data indicated no formation anomalies and established regional background radiation levels. Durango background radionuclide concentrations in surface soil were determined to be 20.3 {plus minus} 3.4 pCi/g for {sup 40}K, 1.6 {plus minus} 0.5 pCi/g for {sup 226}Ra, and 1.2 {plus minus} 0.3 pCi/g for {sup 232}Th. The Durango background gamma exposure rate was found to be 16.5 {plus minus} 1.3 {mu}R/h. Average gamma spectral count rate measurements for {sup 40}K, {sup 226}Ra and {sup 232}Th were determined to be 553, 150, and 98 counts per minute (cpm), respectively. Geologic unit profiles and Durango background radiation measurements are presented and compared with other areas. 19 refs., 15 figs., 5 tabs.« less

  16. Latent variable method for automatic adaptation to background states in motor imagery BCI

    NASA Astrophysics Data System (ADS)

Dagaev, Nikolay; Volkova, Ksenia; Ossadtchi, Alexei

    2018-02-01

Objective. Brain-computer interface (BCI) systems are known to be vulnerable to variabilities in background states of a user. Usually, no detailed information on these states is available even during the training stage. Thus there is a need in a method which is capable of taking background states into account in an unsupervised way. Approach. We propose a latent variable method that is based on a probabilistic model with a discrete latent variable. In order to estimate the model’s parameters, we suggest to use the expectation maximization algorithm. The proposed method is aimed at assessing characteristics of background states without any corresponding data labeling. In the context of asynchronous motor imagery paradigm, we applied this method to the real data from twelve able-bodied subjects with open/closed eyes serving as background states. Main results. We found that the latent variable method improved classification of target states compared to the baseline method (in seven of twelve subjects). In addition, we found that our method was also capable of background states recognition (in six of twelve subjects). Significance. Without any supervised information on background states, the latent variable method provides a way to improve classification in BCI by taking background states into account at the training stage and then by making decisions on target states weighted by posterior probabilities of background states at the prediction stage.

Background The United States' Special Supplemental Nutrition Program for Women, Infants and Children (WIC) distributes about half the infant formula used in the United States at no cost to the families. This is a matter of concern because it is known that feeding with infant formula results in worse health outcomes for infants than breastfeeding. Discussion The evidence that is available indicates that the WIC program has the effect of promoting the use of infant formula, thus placing infants at higher risk. Moreover, the program violates the widely accepted principles that have been set out in the International Code of Marketing of Breast-milk Substitutes and in the human right to adequate food. Summary There is no good reason for an agency of government to distribute large quantities of free infant formula. It is recommended that the large-scale distribution of free infant formula by the WIC program should be phased out. PMID:16722534

Major cardiovascular diseases (CVDs) are leading causes of mortality among US Hispanic and Latino individuals. Comprehensive data are limited regarding the prevalence of CVD risk factors in this population and relations of these traits to socioeconomic status (SES) and acculturation. To describe prevalence of major CVD risk factors and CVD (coronary heart disease [CHD] and stroke) among US Hispanic/Latino individuals of different backgrounds, examine relationships of SES and acculturation with CVD risk profiles and CVD, and assess cross-sectional associations of CVD risk factors with CVD. Multicenter, prospective, population-based Hispanic Community Health Study/Study of Latinos including individuals of Cuban (n = 2201), Dominican (n = 1400), Mexican (n = 6232), Puerto Rican (n = 2590), Central American (n = 1634), and South American backgrounds (n = 1022) aged 18 to 74 years. Analyses involved 15,079 participants with complete data enrolled between March 2008 and June 2011. Adverse CVD risk factors defined using national guidelines for hypercholesterolemia, hypertension, obesity, diabetes, and smoking. Prevalence of CHD and stroke were ascertained from self-reported data. Age-standardized prevalence of CVD risk factors varied by Hispanic/Latino background; obesity and current smoking rates were highest among Puerto Rican participants (for men, 40.9% and 34.7%; for women, 51.4% and 31.7%, respectively); hypercholesterolemia prevalence was highest among Central American men (54.9%) and Puerto Rican women (41.0%). Large proportions of participants (80% of men, 71% of women) had at least 1 risk factor. Age- and sex-adjusted prevalence of 3 or more risk factors was highest in Puerto Rican participants (25.0%) and significantly higher (P < .001) among participants with less education (16.1%), those who were US-born (18.5%), those who had lived in the United States 10 years or longer (15.7%), and those who preferred English (17.9%). Overall, self-reported CHD and stroke

Objective. To examine available data and actions surrounding current pharmacy workforce issues in the United States and United Kingdom. Methods. Published pharmacy workforce data from the United States and United Kingdom were gathered from various sources, including PUBMED, Internet search engines, and pharmacy organization websites. Data was collated from additional sources including scientific literature, internal documents, news releases, and policy positions. Results. The number of colleges and schools of pharmacy has expanded by approximately 50% in both the United States and United Kingdom over the previous decade. In the United States, continued demand for the pharmacy workforce has been forecasted, but this need is based on outdated supply figures and assumptions for economic recovery. In the United Kingdom, workforce modeling has predicted a significant future oversupply of pharmacists, and action within the profession has attempted to address the situation through educational planning and regulation. Conclusion. Workforce planning is an essential task for sustaining a healthy profession. Recent workforce planning mechanisms in the United Kingdom may provide guidance for renewed efforts within the profession in the United States. PMID:25861098

  10. Origin and influence of coal mine drainage on streams of the United States

    USGS Publications Warehouse

    Powell, J.D.

    1988-01-01

Degradation of water quality related to oxidation of iron disulfide minerals associated with coal is a naturally occurring process that has been observed since the late seventeenth century, many years before commencement of commercial coal mining in the United States. Disturbing coal strata during mining operations accelerates this natural deterioration of water quality by exposing greater surface areas of reactive minerals to the weathering effects of the atmosphere, hydrosphere, and biosphere. Degraded water quality in the temperate eastern half of the United States is readily detected because of the low mineralization of natural water. Maps are presented showing areas in the eastern United States where concentrations of chemical constituents in water affected by coal mining (pH, dissolved sulfate, total iron, total manganese) exceed background values and indicate effects of coal mining. Areas in the East most affected by mine drainage are in western Pennsylvania, southern Ohio, western Maryland, West Virginia, southern Illinois, western Kentucky, northern Missouri, and southern Iowa. Effects of coal mining on water quality in the more arid western half of the United States are more difficult to detect because of the high degree of mineralization of natural water. Normal background concentrations of constituents are not useful in evaluating effects of coal mine drainage on streams in the more arid West. Three approaches to reduce the effects of coal mining on water quality are: (1) exclusion of oxygenated water from reactive minerals, (2) neutralization of the acid produced, (3) retardation of acid-producing bacteria population in spoil material, by application of detergents that do not produce byproducts requiring disposal. These approaches can be used to help prevent further degradation of water quality in streams by future mining. ?? 1988 Springer-Verlag New York Inc.

  2. Estimating State-Specific Contributions to PM2.5- and O3-Related Health Burden from Residential Combustion and Electricity Generating Unit Emissions in the United States

Background: Residential combustion (RC) and electricity generating unit (EGU) emissions adversely impact air quality and human health by increasing ambient concentrations of fine particulate matter (PM2.5) and ozone (O3). Studies to date have not isolated contributing emissions by state of origin (source-state), which is necessary for policy makers to determine efficient strategies to decrease health impacts. Objectives: In this study, we aimed to estimate health impacts (premature mortalities) attributable to PM2.5 and O3 from RC and EGU emissions by precursor species, source sector, and source-state in the continental United States for 2005. Methods: We used the Community Multiscale Air Quality model employing the decoupled direct method to quantify changes in air quality and epidemiological evidence to determine concentration–response functions to calculate associated health impacts. Results: We estimated 21,000 premature mortalities per year from EGU emissions, driven by sulfur dioxide emissions forming PM2.5. More than half of EGU health impacts are attributable to emissions from eight states with significant coal combustion and large downwind populations. We estimate 10,000 premature mortalities per year from RC emissions, driven by primary PM2.5 emissions. States with large populations and significant residential wood combustion dominate RC health impacts. Annual mortality risk per thousand tons of precursor emissions (health damage functions) varied significantly across source-states for both source sectors and all precursor pollutants. Conclusions: Our findings reinforce the importance of pollutant-specific, location-specific, and source-specific models of health impacts in design of health-risk minimizing emissions control policies. 15. Chapter 7: Precipitation Change in the United States

    Easterling, D. R.; Kunkel, K. E.; Arnold, J. R.; Knutson, T.; LeGrande, A. N.; Leung, L. R.; Vose, R. S.; Waliser, D. E.; Wehner, M. F.

    2017-01-01

Annual precipitation has decreased in much of the West, Southwest, and Southeast and increased in most of the Northern and Southern Plains, Midwest, and Northeast. A national average increase of 4% in annual precipitation since 1901 is mostly a result of large increases in the fall season. Heavy precipitation events in most parts of the United States have increased in both intensity and frequency since 1901. There are important regional differences in trends, with the largest increases occurring in the northeastern United States. In particular, mesoscale convective systems (organized clusters of thunderstorms)-the main mechanism for warm season precipitation in the central part of the United States-have increased in occurrence and precipitation amounts since 1979. The frequency and intensity of heavy precipitation events are projected to continue to increase over the 21st century (high confidence). Mesoscale convective systems in the central United States are expected to continue to increase in number and intensity in the future. There are, however, important regional and seasonal differences in projected changes in total precipitation: the northern United States, including Alaska, is projected to receive more precipitation in the winter and spring, and parts of the southwestern United States are projected to receive less precipitation in the winter and spring. Northern Hemisphere spring snow cover extent, North America maximum snow depth, snow water equivalent in the western United States, and extreme snowfall years in the southern and western United States have all declined, while extreme snowfall years in parts of the northern United States have increased. Projections indicate large declines in snowpack in the western United States and shifts to more precipitation falling as rain than snow in the cold season in many parts of the central and eastern United States.

  16. State background checks for gun purchase and firearm deaths: an exploratory study.

    2012-10-01

This study examines the relationship between the types of background-information check required by states prior to firearm purchases, and firearm homicide and suicide deaths. Negative binomial models are used to analyze state-level data for homicides and suicides in the U.S. from 1996 to 2005. Data on types of background information are retrieved from the Surveys of State Procedures Related to Firearm Sales, and the violent death data are from the WISQARS. Several other state level factors were controlled for. More background checks are associated with fewer homicide (IRR:0.93, 95% CI:0.91-0.96) and suicide (IRR:0.98, 95% CI:0.96-1.00) deaths. Firearm homicide deaths are lower when states have checks for restraining orders (IRR:0.87, 95% CI:0.79-0.95) and fugitive status (IRR:0.79, 95% CI:0.72-0.88). Firearm suicide deaths are lower when states have background checks for mental illness (IRR:0.96, 95% CI:0.92-0.99), fugitive status (IRR:0.95, 95% CI:0.90-0.99) and misdemeanors (IRR:0.95, 95% CI:0.92-1.00). It does not appear that reductions in firearm deaths are offset by increases in non-firearm violent deaths. More extensive background checks prior to gun purchase are mostly associated with reductions in firearm homicide and suicide deaths. Several study limitations are acknowledged, and further research is called for to ascertain causality. Copyright © 2012 Elsevier Inc. All rights reserved.

    1956-01-01

Reservoir storage facilities in the United States play an important part in the national economy. Storage facilities have enabled the country to utilize to a much fuller extent one of the most valuable natural resources: water. During recent years the construction of reservoirs has continued at a high rate. This report shows the status of these facilities on January 1, 1954, and describes briefly some of the reasons for growth of reservoir facilities in the United States. Descriptive data are given for reservoirs having a capacity of 5, 000 acre-feet or more and for natural lakes having a usable capacity of 5,000 acre-feet or more. Included are reservoirs and lakes completed as of January 1, 1954, and reservoirs under construction on that date. The total number of such reservoirs and lakes is 1, 300. A descriptive list of reservoirs in the United States was first published by the United States Geological Survey in March 1948. That report, Geological Survey Circular 23, entitled Reservoirs in the United States, included reservoirs completed as of January 1, 1947. Since January 1, 1947, reservoirs representing a total usable capacity of 115,000,000 acre-feet, or an increase of 71 percent, have been constructed or are under construction. Data about these new reservoirs are presented herein, and the data shown for reservoirs constructed before 1947 have been corrected on the basis of the latest available survey to determine reservoir capacity. The total usable capacity of reservoirs and lakes included in this compilation amounts to 278, 120, 000 acre-feet, and the corresponding surface area totals 11, 046, 000 acres.

  2. Background radiation in the Albuquerque, New Mexico, U.S.A., area

    NASA Astrophysics Data System (ADS)

    Brookins, Douglas G.

    1992-01-01

Background radiation levels in the Albuquerque, New Mexico, area are elevated when compared to much of the United States. Soil K, U, and Th are somewhat elevated compared to average values in this country and generate roughly 60 mrem per year to the average resident. Cosmic ray contribution, due to the mean elevation of 5,200 ft above sea level, is 80 mrem/yr—well over the average for the United States. Thirty percent of the homes in Albuquerque contain indoor radon levels over the EPA action level of 4 pCi/ℓ compared to 10 12 percent of homes for the entire United States. Indoor radon contributes about 100 300 mrem/yr. Food, beverages, and x-ray doses are assumed at an average-equivalent for the United States and locally yield 96 mrem/yr. Total contributions from other minor sources (color TV, coal, weapons fallout, etc.) are under 10 mrem/yr. Thus total background radiation received by Albuquerque residents is about 330 530 mrem/yr, well in excess of the rest of the United States. The spread in mrem values is due to variations in the contribution from indoor radon.

    2016-01-01

BACKGROUND Thresholds for repair of abdominal aortic aneurysms vary considerably among countries. METHODS We examined differences between England and the United States in the frequency of aneurysm repair, the mean aneurysm diameter at the time of the procedure, and rates of aneurysm rupture and aneurysm-related death. Data on the frequency of repair of intact (nonruptured) abdominal aortic aneurysms, in-hospital mortality among patients who had undergone aneurysm repair, and rates of aneurysm rupture during the period from 2005 through 2012 were extracted from the Hospital Episode Statistics database in England and the U.S. Nationwide Inpatient Sample. Data on the aneurysm diameter at the time of repair were extracted from the U.K. National Vascular Registry (2014 data) and from the U.S. National Surgical Quality Improvement Program (2013 data). Aneurysm-related mortality during the period from 2005 through 2012 was determined from data obtained from the Centers for Disease Control and Prevention and the U.K. Office of National Statistics. Data were adjusted with the use of direct standardization or conditional logistic regression for differences between England and the United States with respect to population age and sex. RESULTS During the period from 2005 through 2012, a total of 29,300 patients in England and 278,921 patients in the United States underwent repair of intact abdominal aortic aneurysms. Aneurysm repair was less common in England than in the United States (odds ratio, 0.49; 95% confidence interval [CI], 0.48 to 0.49; P<0.001), and aneurysm-related death was more common in England than in the United States (odds ratio, 3.60; 95% CI, 3.55 to 3.64; P<0.001). Hospitalization due to an aneurysm rupture occurred more frequently in England than in the United States (odds ratio, 2.23; 95% CI, 2.19 to 2.27; P<0.001), and the mean aneurysm diameter at the time of repair was larger in England (63.7 mm vs. 58.3 mm, P<0.001). CONCLUSIONS We found a lower rate

Background Detailed information regarding plasma use in the United States is needed to identify opportunities for practice improvement and design of clinical trials of plasma therapy. Study Design and Methods Ten US hospitals collected detailed medical information from the electronic health records for 1 year (2010-2011) for all adult patients transfused with plasma. Results A total of 72,167 units of plasma were transfused in 19,596 doses to 9269 patients. The median dose of plasma was 2 units (interquartile range, 2-4; range 1-72); 15% of doses were 1 unit, and 45% were 2 units. When adjusted by patient body weight (kg), the median dose was 7.3 mL/kg (interquartile range, 5.5-12.0). The median pretransfusion international normalized ratio (INR) was 1.9 (25%-75% interquartile range, 1.6-2.6). A total of 22.5% of plasma transfusions were given to patients with an INR of less than 1.6 and 48.5% for an INR of 2.0 or more. The median posttransfusion INR was 1.6 (interquartile range, 1.4-2.0). Only 42% of plasma transfusions resulted in a posttransfusion INR of less than 1.6. Correction of INR increased as the plasma dose increased from 1 to 4 units (p < 0.001). There was no difference in the INR response to different types of plasma. The most common issue locations were general ward (38%) and intensive care unit (ICU; 42%). Conclusion This large database describing plasma utilization in the United States provides evidence for both inadequate dosing and unnecessary transfusion. Measures to improve plasma transfusion practice and clinical trials should be directed at patients on medical and surgical wards and in the ICU where plasma is most commonly used. PMID:25522888

This map and accompanying table show Hydrologic Unites that are basically hydrographic in nature. The Cataloging Unites shown supplant the Cataloging Units previously depicted n the 1974 State Hydrologic Unit Map. The boundaries as shown have been adapted from the 1974 State Hydrologic Unit Map, "The Catalog of Information on Water Data" (1972), "Water Resources Regions and Subregions for the National Assessment of Water and Related Land Resources" by the U.S. Water Resources Council (1970), "River Basin of the United States" by the U.S. Soil Conservation Service (1963, 1970), "River Basin Maps Showing Hydrologic Stations" by the Inter-Agency Committee on Water Resources, Subcommittee on Hydrology (1961), and State planning maps. The Political Subdivision has been adopted from "Counties and County Equivalents of the States if the United States" presented in Federal Information Processing Standards Publication 6-2, issued by the National Bureau of Standards (1973) in which each county or county equivalent is identified by a 2-character State code and a 3-character county code. The Regions, Subregions and Accounting Units are aggregates of the Cataloging Unites. The Regions and Sub regions are currently (1978) used by the U.S> Water Resources Council for comprehensive planning, including the National Assessment, and as a standard geographical framework for more detailed water and related land-resources planning. The Accounting Units are those currently (1978) in use by the U.S. Geological Survey for managing the National Water Data Network. This map was revised to include a boundary realinement between Cataloging Units 10140103 and 10160009.

Background: Major differences exist in residency training, and the structure and quality of residency programs differ between different countries and teaching centers. It is of vital importance that a better understanding of the similarities and differences in plastic surgery training be ascertained as a means of initiating constructive discussion and commentary among training programs worldwide. In this study, the authors provide an overview of plastic surgery training in the United States and Europe. Methods: A survey was sent to select surgeons in 10 European countries that were deemed to be regular contributors to the plastic surgery literature. The questions focused on pathway to plastic surgery residency, length of training, required pretraining experience, training scheme, research opportunities, and examinations during and after plastic surgery residency. Results: Plastic surgery residency training programs in the United States differ from the various (selected) countries in Europe and are described in detail. Conclusions: Plastic surgery education is vastly different between the United States and Europe, and even within Europe, training programs remain heterogeneous. Standardization of curricula across the different countries would improve the interaction of different centers and facilitate the exchange of vital information for quality control and future improvements. The unique characteristics of the various training programs potentially provide a basis from which to learn and to gain from one another. PMID:27257571

Tuberculosis (TB) is a leading public health problem and a recognized priority for the federal Governments of both Mexico and the United States of America. The objectives of this research, primarily for the four states in the United States that are along the border with Mexico, were to: (1) describe the epidemiological situation of TB, (2) identify TB risk factors, and (3) discuss tuberculosis program strategies. We analyzed tuberculosis case reports collected from 1993 through 2001 by the tuberculosis surveillance system of the United States. We used those data to compare TB cases mainly among three groups: (1) Mexican-born persons in the four United States border states (Arizona, California, New Mexico, and Texas), (2) persons in those four border states who had been born in the United States, and (3) Mexican-born persons in the 46 other states of the United States, which do not border Mexico. For the period from 1993 through 2001, of the 16 223 TB cases reported for Mexican-born persons in the United States, 12 450 of them (76.7%) were reported by Arizona, California, New Mexico, and Texas. In those four border states overall in 2001, tuberculosis case rates for Mexican-born persons were 5.0 times as high as the rates for persons born in the United States; those four states have 23 counties that directly border on Mexico, and the ratio in those counties was 5.8. HIV seropositivity, drug and alcohol use, unemployment, and incarceration were significantly less likely to be reported in Mexican-born TB patients from the four border states and the nonborder states than in patients born in the United States from the four border states (P < 0.001). Multivariate analysis revealed that among pulmonary tuberculosis patients who were 18-64 years of age and residing in the four border states, the Mexican-born patients were 3.6 times as likely as the United States-born patients were to have resistance to at least isoniazid and rifampin (i. e., to have multidrug-resistant TB

To examine whether stricter firearm legislation is associated with rates of fatal police shootings. We used a cross-sectional, state-level design to evaluate the effect of state-level firearm legislation on rates of fatal police shootings from January 1, 2015, through October 31, 2016. We measured state-level variation in firearm laws with legislative scorecards from the Brady Center, and for fatal police shootings we used The Counted, an online database maintained by The Guardian. State-level firearm legislation was significantly associated with lower rates of fatal police shootings (incidence rate ratio = 0.961; 95% confidence interval = 0.939, 0.984). When we controlled for sociodemographic factors, states in the top quartile of legislative strength had a 51% lower incidence rate than did states in the lowest quartile. Laws aimed at strengthening background checks, promoting safe storage, and reducing gun trafficking were associated with fewer fatal police shootings. Legislative restrictions on firearms are associated with reductions in fatal police shootings. Public Health Implications. Although further research is necessary to determine causality and potential mechanisms, firearm legislation is a potential policy solution for reducing fatal police shootings in the United States.

A total of 10,000 U.S. households in 25 standard metropolitan statistical areas and 25 counties were included in the United States. More than 8,200 households granted an interview. Nine of every ten households in the United States used some types of pesticide in their house, garden, or yard. Households in the southeastern United States used the most pesticides. Although more than 500 different pesticide formulations were used by the sampled households, 15 pesticides accounted for 65.5% of all pesticides reported in this study. Thirteen of these 15 pesticides were insecticides, one was a herbicide, and one was a rodenticide.

Endemic parasitic infections in the United States are more frequent than is commonly perceived. Intestinal parasitic infection with Cryptosporidium, Dientamoeba, and Giardia occurs most often in children in northern states during the summer months. Zoonotic Toxocara and Toxoplasma parasitic infections are more frequent in southern states, in African Americans, and in populations with lower socioeconomic status. Approximately 300, 000 people in the United States have Trypanosoma cruzi infection. Local, vector-borne transmission of T cruzi and Leishmania infections has been documented in southern states. Parasitic diseases endemic to the United States are not uncommon but are understudied. Copyright © 2013 Elsevier Inc. All rights reserved.

The USGS National Earthquake Information Center (NEIC) has planned and is developing a broadband digital seismograph network for the United States. The network will consist of approximately 150 seismograph stations distributed across the contiguous 48 states and across Alaska, Hawaii, Puerto Rico and the Virgin Islands. Data transmission will be via two-way satellite telemetry from the network sites to a central recording facility at the NEIC in Golden, Colorado. The design goal for the network is the on-scale recording by at least five well-distributed stations of any seismic event of magnitude 2.5 or greater in all areas of the United States except possibly part of Alaska. All event data from the network will be distributed to the scientific community on compact disc with read-only memory (CD-ROM). ?? 1989.

    2011-01-01

BACKGROUND In the southern region of the United States, such as in Louisiana and Texas, there are autochthonous cases of leprosy among native-born Americans with no history of foreign exposure. In the same region, as well as in Mexico, wild armadillos are infected with Mycobacterium leprae. METHODS Whole-genome resequencing of M. leprae from one wild armadillo and three U.S. patients with leprosy revealed that the infective strains were essentially identical. Comparative genomic analysis of these strains and M. leprae strains from Asia and Brazil identified 51 single-nucleotide polymorphisms and an 11-bp insertion–deletion. We genotyped these polymorphic sites, in combination with 10 variable-number tandem repeats, in M. leprae strains obtained from 33 wild armadillos from five southern states, 50 U.S. outpatients seen at a clinic in Louisiana, and 64 Venezuelan patients, as well as in four foreign reference strains. RESULTS The M. leprae genotype of patients with foreign exposure generally reflected their country of origin or travel history. However, a unique M. leprae genotype (3I-2-v1) was found in 28 of the 33 wild armadillos and 25 of the 39 U.S. patients who resided in areas where exposure to armadillo-borne M. leprae was possible. This genotype has not been reported elsewhere in the world. CONCLUSIONS Wild armadillos and many patients with leprosy in the southern United States are infected with the same strain of M. leprae. Armadillos are a large natural reservoir for M. leprae, and leprosy may be a zoonosis in the region. (Funded by the National Institute of Allergy and Infectious Diseases and others.) PMID:21524213

  1. Alcohol Use among Recent Latino Immigrants Before and After Immigration to the United States

    2013-01-01

Background US-born Latinos have higher rates of alcohol use than Latinos who have immigrated to the United States. However, little is known about the pre-immigration drinking patterns of Latino immigrants or about the changes in their drinking behaviors in the 2 years post-immigration. Objectives This article reports findings of a longitudinal study that compared rates of regular, binge, and heavy drinking among a cohort of recent Latino immigrants, ages 18–34, prior to immigration to the United States and in the 2 years post-immigration. Methods Baseline data were collected on the drinking patterns of 405 Latino immigrants living in the United States for 12 months or less. A follow-up assessment occurred during their second year in the United States. Results Findings indicate that number of days of drinking declined significantly post-immigration. Binge alcohol use (five or more drinks on the same occasion during the past 90 days) significantly declined during the post-immigration period. Heavy alcohol use (five or more drinks on the same occasion on five or more days during the past 90 days) also significantly decreased. Conclusions Results suggest a need for continued exploration of pre-immigration drinking patterns and research to uncover underlying factors associated with declines in rates of problematic alcohol use among recent Latino immigrants. Scientific Significance The results of this study can aid in furthering our understanding of the alcohol use of Latino immigrants ages 18–34 prior to and post immigration to the United States to guide future research and the development of culturally tailored clinical interventions. In 1969, CDC began abortion surveillance to document the number and characteristics of women obtaining legal induced abortions, to monitor unintended pregnancy, and to assist efforts to identify and reduce preventable causes of morbidity and mortality associated with abortions. This report summarizes and reviews information reported to CDC regarding legal induced abortions obtained in the United States in 1997. For each year since 1969, CDC has compiled abortion data by state where the abortion occurred. The data are received from 52 reporting areas in the United States: 50 states, the District of Columbia, and New York City. In 1997, a total of 1,186,039 legal abortions were reported to CDC, representing a 3% decrease from the number reported for 1996. The abortion ratio was 306 legal induced abortions per 1,000 live births, and since 1995, the abortion rate has remained at 20 per 1,000 women aged 15-44 years. The availability of information about characteristics of women who obtained an abortion in 1997 varied by state and by the number of states reporting each characteristic. The total number of legal induced abortions by state is reported by state of residence and state of occurrence; characteristics of women obtaining abortions in 1997 are reported by state of occurrence. Women who were undergoing an abortion were more likely to be young (i.e., aged < 25 years), white, and unmarried; approximately one half were obtaining an abortion for the first time. More than one half of all abortions for which gestational age was reported (55%) were performed at < or = 8 weeks of gestation, and 88% were performed before 13 weeks. Overall, 18% of abortions were performed at the earliest weeks of gestation (< or = 6 weeks), 18% at 7 weeks of gestation, and 20% at 8 weeks of gestation. From 1992 through 1997, increases have occurred in the percentage of abortions performed at the very early weeks of gestation. Few abortions were provided after 15 weeks of gestation--4% of

    2016-01-01

Background In 2010, Mexico was the most common (22.9%) country of origin for foreign-born persons with tuberculosis in the United States, and overall trends in tuberculosis morbidity are substantially influenced by the Mexico-born population. Objectives To determine the risk of tuberculosis disease among Mexico-born persons living in the United States. Methods Using data from the U.S. National Tuberculosis Surveillance System and the American Community Survey, we examined tuberculosis case counts and case rates stratified by years since entry into the United States and geographic proximity to the United States–Mexico border. We calculated trends in case rates over time measured by average annual percent change. Results The total tuberculosis case count (−14.5%) and annual tuberculosis case rate (average annual percent change −5.1%) declined among Mexico-born persons. Among those diagnosed with tuberculosis less than 1 year since entry into the United States (newly arrived persons), there was a decrease in tuberculosis cases (−60.4%), no change in tuberculosis case rate (average annual percent change of 0.0%), and a decrease in population (−60.7%). Among those living in the United States for more than 5 years (non-recently arrived persons), there was an increase in tuberculosis cases (+3.4%), a decrease in tuberculosis case rate (average annual percent change of −4.9%), and an increase in population (+62.7%). In 2010, 66.7% of Mexico-born cases were among non–recently arrived persons, compared with 51.1% in 2000. Although border states reported the highest proportions (>15%) of tuberculosis cases that were Mexico-born, the highest Mexico-born–specific tuberculosis case rates (>20/100,000 population) were in states in the eastern and southeastern regions of the United States. Conclusions The decline in tuberculosis morbidity among Mexico-born persons may be attributed to fewer newly arrived persons from Mexico and lower tuberculosis case rates among

    2006-11-15

Purpose: About half of all cancer patients in the United States receive radiation therapy as a part of their cancer treatment. Little is known, however, about the facilities that currently deliver external beam radiation. Our goal was to construct a comprehensive database of all radiation therapy facilities in the United States that can be used for future health services research in radiation oncology. Methods and Materials: From each state's health department we obtained a list of all facilities that have a linear accelerator or provide radiation therapy. We merged these state lists with information from the American Hospital Association (AHA),more » as well as 2 organizations that audit the accuracy of radiation machines: the Radiologic Physics Center (RPC) and Radiation Dosimetry Services (RDS). The comprehensive database included all unique facilities listed in 1 or more of the 4 sources. Results: We identified 2,246 radiation therapy facilities operating in the United States as of 2004-2005. Of these, 448 (20%) facilities were identified through state health department records alone and were not listed in any other data source. Conclusions: Determining the location of the 2,246 radiation facilities in the United States is a first step in providing important information to radiation oncologists and policymakers concerned with access to radiation therapy services, the distribution of health care resources, and the quality of cancer care.« less

Crushed stone and sand and gravel are the two main sources of natural aggregates. These materials are commonly used construction materials and frequently can be interchanged with one another. They are widely used throughout the United States, with every State except two producing crushed stone. Together they amount to about half the mining volume in the United States. Approximately 96 percent of sand and gravel and 77 percent of the crushed stone produced in the United States are used in the construction industry. Natural aggregates are widely distributed throughout the United States in a variety of geologic environments. Sand and gravel deposits commonly are the results of the weathering of bedrock and subsequent transportation and deposition of the material by water or ice (glaciers). As such, they commonly occur as river or stream deposits or in glaciated areas as glaciofluvial and other deposits. Crushed stone aggregates are derived from a wide variety of parent bedrock materials. Limestone and other carbonates account for approximately three quarters of the rocks used for crushed stone, with granite and other igneous rocks making up the bulk of the remainder. Limestone deposits are widespread throughout the Central and Eastern United States and are scattered in the West. Granites are widely distributed in the Eastern and Western United States, with few exposures in the Midwest. Igneous rocks (excluding granites) are largely concentrated in the Western United States and in a few isolated localities in the East. Even though natural aggregates are widely distributed throughout the United States, they are not universally available for consumptive use. Some areas are devoid of sand and gravel, and potential sources of crushed stone may be covered with sufficient unconsolidated material to make surface mining impractical. In some areas many aggregates do not meet the physical property requirements for certain uses, or they may contain mineral constituents that react
