    The article presents the results of analysis of effectiveness of application of main resources in organizing and quality supporting of medical diagnostic care to patients in the clinic of research institute of balneology profile. The result data points out the insufficient effectiveness of application of these resources; the defects in organization and quality of curative diagnostic and rehabilitation care. They determine the priority directions of enhancement of functioning of the institution being a clinical base of research institute of balneology.

  2. [Dr Anto Marić (1897-1982): a journey from alcohology and dermatovenereology to psychiatry and balneology].

    Dr Anto Marić (1897-1982) was born in Vukšić in Bosnia. He completed medical studies in Vienna and Prague. He published his results from the Department for dermatovenerology at Sarajevo State Hospital, Bosnia. He engaged himself in the movement against alcoholism, too. Later he moved to the Neuropsychiatry ward in Belgrade and was appointed manager in a new psychiatry hospital in Kovin, Serbia. For years he had been a community physician in Stanišić in Vojvodina. During the Second World War, he worked in the psychiatric hospital Vrapče at the outskirts of Zagreb, Croatia and after the war he became the head of a thermal spa in Srebrenica, Bosnia. After specialisation in balneology, he came to Rijeka to overlook the reconstruction of a thermal spa near Buzet in Istria. He made use of his long experience in dermatovenerology, neuropsychiatry and balneology to promote the importance of the unity between physical and psychological for maintaining human health.

    In his 1938 Sigma Xi address (subsequently published as J. Chem. Educ. 1939 , 16 , 440-448 ), Oskar Baudisch emphasizes the importance of balneology, the therapeutic use of baths and natural mineral waters. Although some favorable health effects can be attributed to the psychological influences of the spa resort, Baudisch argues that scientific investigations can reveal how the chemical and physical properties of the springs promote specific cures. He gives numerous examples of previous scientific findings, including his own applications of coordination theory and isotope ratio analysis.

  4. Examination of coagulant additives on qualitative composition of selected thermal waters

    On the territory of Poland occur rich deposits of thermal waters. Although the utilisation of these waters is continuously extending, Poland is not exploiting their full geothermal potential due to high investment costs. Thermal waters industry in Poland to date indicates operations within mainly balneology as well as recreation objectives. Higher temperature values of these waters foster a washout in the surrounding rocks resulting in a high concentration of diluted substances which must be often removed. The following study investigates thermal waters from three intakes for which coagulation processes were conducted. Research clearly shows that coagulant additives not impact on the qualitative composition of thermal water, what is very important according to medicinal properties of water. The study results may be further applied as a valuable piece of information for further exploitation in balneology or within the heating sector and other installations.

  5. A proposal for a worldwide definition of health resort medicine, balneology, medical hydrology and climatology

    Health Resort Medicine, Balneology, Medical Hydrology and Climatology are not fully recognised as independent medical specialties at a global international level. Analysing the reasons, we can identify both external (from outside the field) and internal (from inside the field) factors. External arguments include, e.g. the lack of scientific evidence, the fact that Balneotherapy and Climatotherapy is not used in all countries, and the fact that Health Resort Medicine, Balneology, Medical Hydrology and Climatology focus only on single methods and do not have a comprehensive concept. Implicit barriers are the lack of international accepted terms in the field, the restriction of being allowed to practice the activities only in specific settings, and the trend to use Balneotherapy mainly for wellness concepts. Especially the implicit barriers should be subject to intense discussions among scientists and specialists. This paper suggests one option to tackle the problem of implicit barriers by making a proposal for a structure and description of the medical field, and to provide some commonly acceptable descriptions of content and terminology. The medical area can be defined as “medicine in health resorts” (or “health resort medicine”). Health resort medicine includes “all medical activities originated and derived in health resorts based on scientific evidence aiming at health promotion, prevention, therapy and rehabilitation”. Core elements of health resort interventions in health resorts are balneotherapy, hydrotherapy, and climatotherapy. Health resort medicine can be used for health promotion, prevention, treatment, and rehabilitation. The use of natural mineral waters, gases and peloids in many countries is called balneotherapy, but other (equivalent) terms exist. Substances used for balneotherapy are medical mineral waters, medical peloids, and natural gases (bathing, drinking, inhalation, etc.). The use of plain water (tap water) for therapy is called hydrotherapy, and the use of climatic factors for therapy is called climatotherapy. Reflecting the effects of health resort medicine, it is important to take other environmental factors into account. These can be classified within the framework of the ICF (International Classification of Functioning, Disability and Health). Examples include receiving health care by specialised doctors, being well educated (ICF-domain: e355), having an environment supporting social contacts (family, peer groups) (cf. ICF-domains: d740, d760), facilities for recreation, cultural activities, leisure and sports (cf. ICF-domain: d920), access to a health-promoting atmosphere and an environment close to nature (cf. ICF-domain: e210). The scientific field dealing with health resort medicine is called health resort sciences. It includes the medical sciences, psychology, social sciences, technical sciences, chemistry, physics, geography, jurisprudence, etc. Finally, this paper proposes a systematic international discussion of descriptions in the field of Health Resort Medicine, Balneology, Medical Hydrology and Climatology, and discusses short descriptive terms with the goal of achieving internationally accepted distinct terms. This task should be done via a structured consensus process and is of major importance for the publication of scientific results as well as for systematic reviews and meta-analyses.

  6. Max Hirsch (1875–1941): His forgotten fate and his contributions to the founding of modern rheumatology

    Objective To elucidate the connections between balneology and rheumatology in the founding period of the discipline of rheumatology, and describe the contributions of Max Hirsch, MD in the formation of professional rheumatology societies. Material and Methods Historical documents from the medical history collection of Vogelsang–Gommern, Germany, and original personal documents of the Hirsch family and information from the medical and historical period literature were used in developing this report. Results The first efforts at organizing rheumatology as a recognized clinical and academic discipline took place in the 1920s. Many of the first proponents were balneologists who cared for patients with chronic arthritic conditions without the benefit of effective medications. Max Hirsch, MD was a major figure in the development of modern rheumatology as it emerged from the provenance of balneology and orthopedics as a recognized organized medical discipline, contributing to the founding of the German Society for Rheumatology and the International League Against Rheumatism. Conclusion Max Hirsch made significant contributions to scientific and organized rheumatology in the early days of the discipline. His contributions to the field and his fate as a Jewish physician have only recently come to light. PMID:27733939

  8. Abnormal changes in the density of thermal neutron flux in biocenoses near the earth surface.

    We revealed an increase in the density of thermal neutron flux in forest biocenoses, which was not associated with astrogeophysical events. The maximum spike of this parameter in the biocenosis reached 10,000 n/(sec x m2). Diurnal pattern of the density of thermal neutron flux depended only on the type of biocenosis. The effects of biomodulation of corpuscular radiation for balneology are discussed.

    The simple message of this paper is that it is the high time to reevaluate the strategies and optimize the efforts for investigation of thermal (spa) waters. Several articles trying to clear mode of action of medicinal waters have been published up to now. Almost all studies apply the unproven hypothesis, namely the inorganic ingredients are in close connection with healing effects of bathing. Change of paradigm would be highly necessary in this field taking into consideration the presence of several biologically active organic substances in these waters. A successful design for experimental mechanistic studies is approved.

  10. Nano- and microstructural disperse rocks in protective barriers, medicine and balneology

    On the base of results of electron microscopy, thermogravimetric, X-ray, rheological, mechanochemical and medico-biological research methods, there are proposed models of physico-mechanical, nanochemical, colloidal and biocolloid metamorphic processes of iron oxide-silicate rocks, which are accompanied by formation of nano- and microdispersed pelitic sediments, peloids (therapeutic muds), clays, sedimentary iron-silicate-carbonate ore materials. The role of microorganisms and surface-active products of their vital activity in these processes is shown. It was noted that a stable existence of ecosystems, which contain iron oxide-hydroxide-silicate polymineral formations, is largely determined by preliminary processes of geomechanical dispersion of rocks and by subsequent processes of vital activity of various microorganisms. The metabolic products of such microorganisms activate the biocolloid interactions, which are due to cooperative colloidal, biological, biochemical and nanochemical transformations of biogeocenosis of living and non-living substance. The conceptions of role and meaning of chemical and biocolloidal processes of iron and silicon nanocluster formation are developed for strength changes in contact zones of microparticles in polymineral dispersion systems which, respectively, have an influence on their rheological properties. It is shown that in the presence of sodium chloride (seawater, lake's brine) for low-iron clay-sand systems, a dilatant-thixotropic nature of the flow is observed, and at a high-iron content in the form of nanostructured goethite, a hypernomalous growth of concentrated iron-silicate suspensions' viscosity and enhancement of contact interactions in them are observed. Taking into account the established phenomena the application of iron oxide-silicate peloid compositions in the construction of protective barriers, in medicine and balneology (treatment of injured joints, wounds, leukemia, etc.) is considered.

  11. [The role of balneology in plastic surgery].

    Balneology can be part of the plastic surgery care sector. The objectives of this study were firstly to the state of knowledge about the hydrotherapy and specify the place reserved for hydrotherapy by surgeons as an adjunct in plastic and reconstructive surgery (adult and child). Multicentric national study by poll (Google Drive®) focused at plastic and/or pediatric surgeons. The following information was analyzed: frequency, timing of prescription, indications, the surgeon's feelings towards hydrotherapy and the differences between adult's and children's prescriptions. Fifty-four teams were contacted: 22 responses were received (15 "adult" plastic surgeons, 9 "pediatric" plastic surgeons, 6 pediatric surgeons, with 12 out of 22 working with burnt patients). Eighteen out of 22 prescribed hydrotherapy. Twenty out of 22 thought that hydrotherapy had a role as adjuvant therapy in plastic surgery. The indications were: burns (11/20), skin-graft hypertrophy (10/20), inflammatory and pruritic scar and cutaneous trophic disorders (9/20), psychological (3/20), retractions (2/20), weight loss and smoking (1/20). The timing of the prescription was: < 3 months (2/20), < 6 months (7/20), > 6 months and < 1 year (15/20), > 1 year (8/20) after surgery/trauma. Twenty out of 22 found a beneficial effect: physical (19/20): reduction of inflammatory signs, pruritus and pain, scar maturation, skin thinning improvement; psychological (14/20): positive for patient/family. Five out of 17 made the difference between child/adult, 10/17 made no difference but only treated adults or children. The respondents in the study are probably more sensitive to the effects of hydrotherapy that non-respondents. It is difficult to assess the real impact of hydrotherapy in plastic surgery because distinguishing spontaneous favorable evolution of a scar from one only due to the hydrotherapy or multidisciplinary management is difficult.   12. Radon in thermal waters in south-east part of Serbia.

    There are several occurrences of thermal waters in the south-eastern part of Serbia, which are originating from igneous and metamorphic rocks. These waters are mainly used in balneology, but some of them are used for drinking purposes and in water supply to heat buildings, for greenhouses and to irrigate land. In this region, there is the well-known Niška banja spa, which has elevated levels of radon. Water samples were examined from other spas in the south-eastern part of Serbia in order to determine radon activity concentration. A detailed discussion of a possible correlation between determined radon activity concentration and the geology of this area is also given. © The Author 2014. Published by Oxford University Press.     In this review, the most important complementary und alternative therapies are discussed, focusing particularly on their use in otorhinolaryngology. These therapies include balneology, Kneipp therapy, microbiological therapy, fasting, excretion therapy, different oxygen therapies, hydro-colon therapy, urine therapy, own-blood therapy, Bach therapy, orthomolecular therapy, order therapy, environmental medicine, phytotherapy, homeopathy, complex homeopathy, anthroposophy, neural therapy, electroaccupuncture according to Voll and similar therapies, nasal reflex therapy, reflex-zone massage, manual therapy, massage, lymph drainage, aroma therapy, thermotherapy, bioresonance, kinesiology, hopi candles, and dietetics. Some of these methods and regimens can be recommended, but others should be rejected. In universities, these methods are only represented to a minor extend, but are more accepted by otorhinolaryngologists in practice. This paper provides a guide to which alternative therapies are sensible and possible in otorhinolaryngology. The aim is to stimulate interest in these methods. It is necessary to discuss these alternative methods reasonably and credibly with patients.

    This article is devoted to the topical problems of pediatric balneotherapy with special reference to the organization of the spa and health resort-based treatment of the children in the Russian Federation. The main issues discussed by the authors include the current state of health resort care for the children, the problem of statutory regulation of the activities of the children's spa and health resort facilities, the approaches to increasing the availability of the spa and health resort-based treatment for the children at the enhanced risk of the development of chronic diseases, disabilities, and tuberculosis. Also considered are the problems of the development of the regulatory framework for the medical rehabilitation of the children based at the spa and health resort facilities. The principal goals to be sought in climatotherapy, physiotherapy, balneotherapy, and pelotherapy in the pediatric context are outlined along with the further prospects for the development of the main areas of pediatric balneology.

  15. Sapropel as a Binder: Properties and Application Possibilities for Composite Materials

    Recent development trends largely look for possibilities of a wider use of natural materials and local resources. In this perspective, the use of organic rich lake sediment - sapropel - as a binding material in line with other environmentally friendly filling materials can be considered as a challenge. Sapropel itself is a valuable resource with multiple areas of application, for example, medicine, veterinary, agriculture, livestock farming, balneology, cosmetic applications, construction, and its application options have been widely studied in the 20th century in the Baltic countries, Ukraine and Russia. Birch wood fibre and sanding dust, hemp shives, ‘Aerosil’ are used as a filler and three types of sapropel are used as a binder in making composites. After material preparation and curing, physical and mechanical properties - density, thermal conductivity, compressive and flexural strength, were determined and compared to the data in the literature, and the opportunities to use them in the ecological construction were considered. The obtained results give insight into possibilities to use sapropel as a raw material, which can be considered as prospective material for construction materials and design products.

    Oradea is located on the Crisul Repede River, in the northwestern corner of Romania almost due west of Budapest, Hungary. The city has a population around half a million people and can trace its origins back to the Neolithic Age. It was an urban settlement beginning in the 13th century and has been an economic and cultural center for the region. It is a geothermal city with 12 wells drilled within the city limits, six in the nearby Felix Spa and five in the Bors geothermal area to the west, with one doublet set at Nufarul. Currently, there are amore » variety of geothermal uses in the area, including space and greenhouse heating, domestic hot water supply, process heat, balneology and swimming pools. Wellhead temperatures range from 70 to 105{degrees}C with artesian flows of from 5 to 25 L/s. The present installed capacity is 25 MWt and the heat supply is estimated at 60,000 MWh per year (216,000 GJ/yr). With pumping, the production would double and adding four more doublets, the installed capacity be increased to 65 MWt.« less

  17. Sulphur and skin: from Satan to Saddam!

    Since the dawn of time, Beelzebub has been showering fire and brimstone (sulphur) on tortured souls, but the cutaneous effects of this have been poorly described. Sulphur has also been used for centuries as a treatment for many skin conditions, such as fungal infections, scabies, psoriasis, eczema and acne. It has also been used extensively in cosmetic preparations and by cosmetic dermatologists treating conditions such as seborrhoeic eczema. Many natural bathing spas have high levels of sulphur; such balneology has been advocated by medical and cosmetic dermatologists as an effective treatment for cutaneous disorders for more than 500 years. Sulphur was often the active agent in many of the so-called 'patent medicines' that became popular in the mid-nineteenth century. Time has not withered medical practitioners' enthusiasm for sulphur. There are various reports in the medical literature of its current use. However sulphur treatment is not without its risks; a sulphur spring dermatitis has been described from a spa bath in Taiwan. With the satanic threat of bio-terrorism, some dermatologists may be treating the effects of contact with sulphur mustard all too soon.

  18. Health tourism in a Czech health spa.

    This paper is about the changing shape of health tourism in a Czech spa town. The research focuses on balneotherapy as a traditional Czech healing technique, which involves complex drinking and bathing therapies, as it is increasingly being incorporated into the development of a Czech health tourism industry. Today, the health tourism industry in Mariánske Lázne is attempting to 'harmoniously' combine three elements--balneology, travel and business activities. One detects subtle shifts and consequent incongruities as doctors struggle for control over the medical portion of spa hotels. At the same time, marketing groups are creating new packages for a general clientele, and the implementation of these new packages de-medicalizes balneotherapy. Related to the issue of the doctor's authority in the spa, the changes occurring with the privatization of tourism entails the entrance of 'tourists' to Mariánske Lázne who are not necessarily seeking spa treatment but who are still staying at spa hotels. There is a general consensus among spa doctors and employees that balneotherapy has become commodified. Thus, while balneotherapy remains a traditional form of therapy, the commercial context in which it exists has created a new form of health tourism.

  19. New approaches to estimation of peat deposits for production of biologically active compounds

    It is known, that biologically active preparations from peat increase animals productivity as well as resistance against stress-factors and have adaptogeneous, antioxidant, immunomodulative properties. Optymal choice of peat deposits for the production of biologically active preparations supposes the detailed comparative analysis of peat properties from different deposits. For this the cadastre of peat of Ukraine is developed in the humic substances laboratory named after prof. Khristeva L.A. (Dnipropetrovsk Agrarian University, Ukraine). It based on the research of its physical and chemical properties, toxicity and biological activity, and called Biocadastre. The Biocadastre is based on the set of parameters, including the descriptions of physical and chemical properties (active acidity, degree of decomposition, botanical composition etc.), toxicity estimation (by parabyotyc, infusorial, inhibitor and other tests), biological activity indexes (growth-promoting, antioxidative, adaptogeneous, immunomodulative antistress and other actions). The blocks of Biocadastre indexes are differentiated, taking into account their use for creation the preparations for vegetable, animals and microorganisms. The Biocadastre will allow to choose the peat deposits, most suitable for the production of different biologically active preparations, both wide directed and narrow spectrum of action, depending on application fields (medicine, agriculture, veterinary medicine, microbiological industry, balneology, cosmetology).

  20. [The comprehensive approach to the rehabilitative treatment of junior athletes].

    The objective of the present study was the development of the comprehensive program for the medico-psychological follow-up of the male and female junior athletes (rhythmic gymnastics) with the purpose of stabilizing their hormonal and emotional status, as well as improving sport performances based on the use of modern hardware-software technologies. The comprehensive examination of 72 female athletes at the mean age of 11.5±0.6 years attending R. Kuznetsov specialized school of rhythmic gymnastics of the Olympic reserve in the city of Seversk has been undertaken on the basis of Tomsk Research Institute of Balneology and Physiotherapy, the branch of Siberian Federal Research and Clinical Centre. The program of comprehensive medical psychological rehabilitation for the junior athletes of either sex engaged in sportive activities requiring precise technical actions has been elaborated. The method of the combined therapeutic treatment included physical exercises therapy, manual massage, dry carbonic bathtubs, psychological activities with the application of biological feedback trainings and cognitive trainings; it was intended for the correction of the hormonal status and the improvement of the psycho-emotional conditions of the athletes. The combined treatment based on the use of the modern hardware-software technologies was shown to promote the restoration and development of the psychophysical and psychological qualities of the male and female junior athletes indispensable for the maintenance of their high readiness for the efficient sports activities at the subsequent stages of the training cycle.   1. Biometeorology in austria: past, present and future

    Biometeorology in Austria has been shaped by concepts, personalities, and technology. In early times, the branches of biometeorology that are usual today were already evident: agricultural and forest meteorology, phenology, medical biometeorology and balneology, aerial biometeorology, urban housing and stabling meteorology all started to emerge several centuries ago. From the 1920 up to 1936, Wilhelm Schmidt at the Agricultural University of Austria laid the foundations of modern biometeorology. He was followed by Franz Sauberer, who headed a Department of Biometeorology at the National Weather Service and devoted his active life totally to biometeorology. Several years after his untimely death, the Department was dissolved. Not until 1981 was biometeorology taken up again at the Agricultural University, where the tradition of Schmidt and Sauberer now lives on in several courses within the area of applied biometeorology: Micro-and Topoclimatology, Agricultural and Forest Meteorology and Atmospheric Radiation. Biometeorology, being an experimental science, has also been influenced by new technological developments. The early period was exclusively observational. During the late nineteenth and early twentieth centuries mechanical and simple electric instruments were used with strip-chart recorders. These time consuming methods have now been replaced by electronic devices, including data loggers and portable computers along with many new electronic sensors, which provide additional insight into biometeorological problems. Since computers also make it possible to solve some of the complicated equations of biometeorology, the future of this science seems to be bright, not only in Austria but throughout the world.

  2. Geochemical variables as plausible aetiological cofactors in the incidence of some common environmental diseases in Africa

    Over the last two decades, there has been a rapid growth in research in the field of medical geology around the world, and we continue to encounter “new” and important correlations between certain environmental health conditions and factors related to our interactions with geological materials and processes. A review of the possible role of geochemical factors such as the circulation of Mg, Se and F and the physico-chemical composition of volcanic soil particles, on the aetiology of some common diseases in Africa, is presented. Such studies till now, have tended to emphasise only the deleterious health impacts due to geoenvironmental factors. This is justifiable, since a proper understanding of the negative health impacts has contributed significantly towards improvement in diagnosis and therapy. But there are also beneficial effects accrued from judiciously exploiting geological materials and processes, exemplified in this review, by the several important medical applications of African clays, the therapeutic gains associated with hot springs, and balneology of peat deposits. The criticality of the “optimal range” of intake for the nutrient elements Mg, Se and F in metabolic processes is also emphasised, and illustrations given of illnesses such as cardiovascular disorders and various cancers (all major causes of mortality in Africa) that can possibly occur on either side of this range. It is hoped that this review would help generate ideas for the formulation of experimental studies that take into account the role of the geochemical environment, in an attempt to establish precisely the obscure aetiology of some of the diseases treated, and uncover new pathways in their pathogenesis.

  3. Modelling geothermal conditions in part of the Szczecin Trough - the Chociwel area

    The Chociwel region is part of the Szczecin Trough and constitutes the northeastern segment of the extended Szczecin-Gorzów Synclinorium. Lower Jurassic reservoirs of high permeability of up to 1145 mD can discharge geothermal waters with a rate exceeding 250 m3/h and temperatures reach over 90°C in the lowermost part of the reservoirs. These conditions provide an opportunity to generate electricity from heat accumulated in geothermal waters using binary ORC (Organic Rankine Cycle) systems. A numerical model of the natural state and exploitation conditions was created for the Chociwel area with the use of TOUGH2 geothermal simulator (i.e., integral finite-difference method). An analysis of geological and hydrogeothermal data indicates that the best conditions are found to the southeast of the town of Chociwel, where the bottom part of the reservoir reaches 3 km below ground. This would require drilling two new wells, namely one production and one injection. Simulated production with a flow rate of 275 m3/h, a temperature of 89°C at the wellhead, 30°C injection temperature and wells being 1.2 km separated from each other leads to a small temperature drop and moderate requirements for pumping power over a 50 years' time span. The ORC binary system can produce at maximum 592.5 kW gross power with the R227ea found as the most suitable working fluid. Geothermal brine leaving the ORC system with a temperature c. 53°C can be used for other purposes, namely mushroom growing, balneology, swimming pools, soil warming, de-icing, fish farming and for heat pumps.

  4. From sulphur to perfume: spa and SPA at Monchique, Algarve.

    In the thermal village of Monchique, Algarve, different streams of water-related knowledge and practices coexisted for centuries. Those waters were traditionally known as aguas santas (holy waters) and believed to have redemptive healing powers. In the seventeenth century, the Catholic church took control of the place, refashioned the bathing rituals, developed infrastructures and provided assistance to the patients, granting free treatment to the poor. In the nineteenth century, the state replaced the church and imposed that treatments should be provided by professionals trained in the scientific principles of medical hydrology. Secular and scientific as they were, clinical logbooks still allowed for the account of patients that embodied miracle-like redemptive cures 'at the third bath'. People went to Monchique both for its magic and its medicine, bringing in the body ailments achieved in their lives of hard labour. They also went there for a socialising break while healing. From mendicants to rich landowners, coming mostly from the Algarve and neighbouring Alentejo, they crowded the place in summertime. In the twentieth century, as in other places in continental Europe, the spa evolved into a highly medicalised place that qualified for medical expenses reimbursements, which implied the eclipsing--at least from representation--of its leisure component. In the twenty-first century, a new trend of consumer-centred, market-based, post-water balneology with an emphasis on wellness and leisure reinvented the spa as place for lush and diversified consumption. This article argues that the seemingly contradictory systems (markets and medicine) coexist much in the same way that magic, religion and medicine coexisted in the old water sites. The new SPAs, rather than putting an end to the old spas, have enabled them to survive by reinventing thermal sites as places of attraction and leisure.

  5. Assessment of sulfurous springs in the west of Iraq for balneotherapy, drinking, irrigation and aquaculture purposes.

    This research deals with the sulfurous spring waters flow along the course of the Euphrates River in western Iraq in the area extended between Haqlaniya and Hit within the Al-Anbar governorate. Eleven springs (3 in Haqlanya, 4 in Kubaysa and 4 in Hit) have been addressed for the purpose of water evaluation for balneology, drinking, irrigation and aquaculture (fish farming). In order to meet the objectives of this research, all springs were sampled and analyzed for the total dissolved solid, electrical conductivity, pH, temperature, major cations (Ca(2+), Mg(2+), Na(+) and K(+)), major anions (SO(4)(2-), Cl(-), HCO(3)(-) and CO(3)(2-)), minor anions (PO(4)(3-)and NO(3)(-)) as well as the trace elements that included Pb, Zn, Cd, Ni, Fe, Mn, Cu, Br, F, Ba, B, Sr, Al, As, Cr, Hg and Se. The International Standards of World Health Organization are used for assessing the water quality. The results revealed that the springs belong to the tepid springs of 27-30 °C and classified as hypothermal to the thermal springs. Lithochemistry and geochemical processes clearly affected the water chemistry. The hydrogeochemical processes are responsible for the element enrichment in water by the chemical dissolution of carbonate and gypsum and evaporation as well. The results of the study indicate the possibility of using spring water for therapeutic purposes, but not allowed for drinking and aquaculture (fish farming), except those free of H(2)S gas. On the other hand, it can be used for irrigation with risk. However, soil type as well as proper selection of plants should be taken into consideration.

  6. Risk Assessment of Mineral Groundwater Near Rogaška Slatina

    Groundwater resources of mineral and thermo-mineral water are invaluable for planning a sustainable spatial and economic development of the Rogaška Slatina area, which requires a protection of this natural heritage. Numerous previous investigations of Rogaška groundwaters were subjects to balneology and to demands for larger exploitation quantities, that is why information are missing that are essential for definition of the Rogaška fractured aquifer system with mineral and thermo-mineral water and for its protection. The isotopic investigations of groundwaters stored in the Rogaška Slatina fractured aquifer system were performed aiming at answering open questions on the groundwater recharge and dynamics, on connections between different types of aquifers and on solute transport. Environmental isotopes 2H, 18O, 3H, 13C of dissolved inorganic carbon and 14C were analysed in mineral, thermo-mineral and spring waters. Results indicated the source and mechanism of groundwater recharge, its renewability, a transit time distribution, hydraulic interrelationships, the groundwater origin and its evolution due to effects of water-rock interaction. The mean residence time estimates of mineral and thermo- mineral water in the aquifer are between 3400 and 14000 years. On the other hand, the mixing processes between younger and older waters or mineral and spring waters are reflected as well as waters that infiltrated predominantly after the 1960s. These suggest the vulnerability of the research systems to man-made impacts. The presented results coupled with available information on a physical hydrogeology and water chemistry asses the optimal balance between the environmental protection and economic use of mineral water resources in the study area. They are essential for the protection strategy development of mineral and thermo-mineral water in the Rogaška Slatina area bringing together the state administration and local authorities and stakeholders.

    Thermal water exploitation is widespread, because it is considered to a "green" renewable energy source, the transporter of the Earth crust's heat. It is suitable for very diverse purposes: balneology, heating, mineral water, municipal hot water supply, technological water, etc. After usage, large amount of thermal water becomes sewage water with high concentrations of salts, heavy metals, ammonia, nitrate, and high temperature. Besides that, most of these waters have an unfavourable ion composition. Na+ (and in some cases Mg+) is predominant among cations. A common way of treatment is to let off the waste thermal water in unlined ground channels to leak into the soil. This can cause physical and chemical soil degradation. Continouos Na+ supply occurs, that occupies the place of Ca2+ on the ion exchange surfaces. Thus, adverse effects of Na+ can appear, like formation of extreme moisture regime, peptization, liquefaction. Beside Na+, Mg2+ also helps the formation of physical degradation in the soil. High water retain and unfavourable structure evolves. Not only the physical features of the soil are touched, fertility of production sites as well. Namely sorrounding the unlined ground channels, agricultural areas are seated, so it is important to protect productivity of the soil to maintain yield. Because of the seepage of high salt concentration waters, salt accumulation can be observed near to the channel lines. The investigated sample sites are located in the Great Hungarian Plane. We determined the main pollutants of the thermal waters, and the effects to the sorrounding soils. On two selected investigation areas (Cserkeszőlő, Tiszakécske) salt profiles and Na+ adsorption isotherms are presented to characterize soil degradation. Genetic soil types are differ on the investigated areas, so the aspect of impact is different, as well.

    This investigation aims not only to gain greater insight into the ancient uses of natural heat and its by-products, but also to gather elements for comprehending what kind of impact the presence of geothermal manifestations and the occurrence of volcanic eruptions may have produced on the ancient inhabitants of the Mediterranean and Mesoamerican regions. The first part of the paper discusses what may have occurred in the time period from the Lower Paleolithic (10[sup 5]--10[sup 6] years ago) until the end of the Neolithic. Throughout this period, the relationship of man with the various manifestations of terrestrial heat and itsmore » associated products was quite close and intense. In addition to the initial development of direct uses, this relationship with geothermal energy also involved man's cultural sphere. The second part of the paper discusses the development of direct uses and the importance that thermal balneology attained in some regions of the Mediterranean area in historical times. The exploitation and processing of hydrothermal products by the Etruscans, the blossoming of balneotherapy and the multiple functions of the spas in Roman times, the decline of all direct uses between the 5th and 6th centuries A.D. following the collapse of the Roman Empire, and the intensive exploitation of the manifestations of Larderello between the 11th and 16th centuries are discussed. The third part of the work refers to the Mesoamerican area (Mexico and neighboring regions) and covers the period extending from several millennia before the Christian era until the time of the voyages of Columbus.   9. Chemical and isotopic composition of the Monfortinho thermal water (Portugal): contribution to the aquifer conceptual model and resource evaluation

    Groundwaters from quartzite aquifers are usually cold waters with very low mineralization as consequence of circulation in fractured aquifers and rocks with very low solubility. In the Monfortinho, Beira Baixa region in Portugal, a thermal water occurs associated to a Ordovician quartzite syncline, the Penha Garcia syncline (Sequeira et al., 1999). The thermal water is used for balneology and supplies a thermal Spa trough boreholes discharging about 36 l/s. The syncline of Penha Garcia has NW-SE axis and is fractured by a NE-SW fault, where the valley of Ponsul river is developed. The natural discharge of the thermal aquifer occur at the SE edge of the syncline. The Monfortinho thermal water has temperature around 30 °C, pH of 5.45, very low mineralization, with electric conductivity about 35 uS/cm; the main dissolved specie is the SiO2 that reaches 24 mg/L, corresponding to 53% of the total dissolved solids. The chemical facies is of Na-HCO3 type. The d18O and d2H diagram indicates that Monfortinho water is derived from the local meteoric waters. The δ18O and δ2H content also pointed out a recharge area of the thermal aquifer above 400 m of elevation, with a isotopic gradient value of -0.15‰ d18O/100m. This elevation corresponds to the top of the eastern block of the syncline, suggesting that Ponsul fault is a negative barrier to groundwater flow and the thermal aquifer is developed only in eastern block of the syncline. The groundwater flows at about 600-700 m depth along the syncline base toward SE. The average rainfall in the region is 790 mm/year and the estimated recharge is about 17% (Carvalho, 2001) of the precipitation, corresponding to 134 mm/year and 4x105 m3/ano of hydrothermal resource.   10. Chemical properties of peat used in balneology

    The physiological activity of peats is observed in human peat-bath therapy and in the promotion of growth in some plants. Balneological peat as an ecologically clean and natural substance is perceived as being more 'human friendly' than synthetic compounds. Poland has a long tradition of using balneological peat for therapeutic purposes. Balneological peat reveals a physical effect by altering temperature and biochemical effects through biologically active substances. It is mainly used for the treatment of rheumatic diseases that are quite common in Poland. Peat represents natural product. Physico-chemical properties of peat in particular surface-active, sorption and ion exchanges, defining their biological function, depend mainly on the chemical composition and molecular structure of humic substances representing the major constituent of organic soil (peat). The carbon of organic matter of peats is composed of 10 to 20% carbohydrates, primarily of microbial origin; 20% nitrogen-containing constituents, such as amino acids and amino sugars; 10 to 20% aliphatic fatty acids, alkanes, etc.; with the rest of carbon being aromatic. For balneology peat should be highly decomposed (preferably H8), natural and clean. The content of humic acids should exceed 20% of dry weight, ash content will be less than 15 15% of dry weight, sulphur content less than 0.3% of dry weight and the amount of water more than 85%. It will not contain harmful bacteria and heavy metals. Humic substances (HS) of peat are known to be macromolecular polydisperse biphyllic systems including both hydrophobic domains (saturated hydrocarbon chains, aromatic structural units) and hydrophilic functional groups, i. e having amphiphilic character. Amphiphilic properties of FA are responsible for their solubility, viscosity, conformation, surfactant-like character and a variety of physicochemical properties of considerable biologically practical significance. The chemical composition of peats depends significantly on the genesis of peatlands and the depth of sampling. The chemical properties of peat fulvic acids (FA) have some genetic peculiarities due to the specific conditions of the process of humification of peat-forming plants in mires. The process of humification in mires takes place in the top-forming layer under amphibious moisture conditions. Substances of microbial origin are water-soluble and can participate in the formation of peat FA to a little extent. So a main source of structural units for the peat HA and FA is suggested to be organic constituents of peat forming plants of various botanical composition. The content of aromatic units in peat FA was shown to depend on the content of lignin in peat-forming plants and also of the aromatization of polysaccharides mainly due to the transformation of cellulose. FA characterized lower than humic acids molecular weight (1000-30,000). FA's are composed of a series of highly oxidized aromatic rings with a large number of side chains. Building blocks are benzene carboxylic acids and phenolic acids. These are held together by hydrogen bonding van der Waals' forces and ionic bonding. FA contains larger concentrations of nitrogen. This fraction also contains a great deal of polysaccharide materials, as well as low molecular fatty acids and cytoplasmic constituents of microorganisms. These compounds are linear, flexible colloids at low concentrations, and spherical colloids at high solution concentrations and low pH values. A more adequate knowledge of the chemical structure of humic materials will assist us in better understanding the physiological effects and also the function of these macromolecules on the health that these materials are know to exert. This improved knowledge provides us better information on chemical structure of humic substances from peats, which are responsible for pharmacotherapeutic, pharmacokinetic and biopharmaceutical effect. This structure of FA creates proper conditions for uptake of nutrient as well as bioavailability of biologically active substances. The solubilization in water by humic materials of organic substances which are otherwise water-insoluble is a matter of considerable interest to chemist deals with the problem of the function of organic matter. There has been considerable evidence that humic substances can "complex" with several biologically active substances and so modify their physiological activity. It has been noteworthy that FA can "fix" high-molecular weight water-insoluble organic compounds and make them water-soluble. FA may so act as a vehicle for the mobilization, transport and immobilization of such substances in physiological conditions. Analysis of HA and FA carried out by several analytical methods revealed that there were no chemical interaction among biologically active substances but that latter was firmly adsorbed, possible by hydrogen-bonding, on the FA surfaces. Amino acids account for the majority of organic N fraction in humic substances. Most of the amino acids in organic matter occur in bound form in the humino-peptides fraction. These amino acids are commonly bound to the central core of FA. These humino-peptides fraction of FA mediate in respiration and act as hydrogen acceptors, thus affecting oxidation-reaction reactions. Thus, what is needed at this time is more fundamental research in order to solve practical pharmacological, pharmacokinetic and biopharmaceutical problem of great significance for human health.

    The aims of this study were to conduct a systematic literature review on balneotherapy about the specific therapeutic role of mineral elements and other chemical compounds of mineral waters and derivate peloids/muds and to discuss the study methods used to evaluate it (in musculoskeletal conditions). We searched Medline by PubMed using the following key words: "spa therapy" "balneotherapy" "mud" "peloid" "mud pack Therapy" in combination with "randomized controlled trial" "double blind trial." We also reviewed the reference list of articles retrieved by the Medline search. We selected the double-blind randomized clinical trials that assessed the effects of mineral water or mud treatments compared to tap water, attenuated peloid/mud therapy or similar treatments without the specific minerals or chemical compounds of the treatment group ("non-mineral"). We evaluated the internal validity and the quality of the statistical analysis of these trials. The final selection comprised 27 double-blind randomized clinical trials, 20 related to rheumatology. A total of 1118 patients with rheumatological and other musculoskeletal diseases were evaluated in these studies: 552 of knee osteoarthritis, 47 of hand osteoarthritis, 147 chronic low back pain, 308 of reumathoid arthritis, and 64 of osteoporosis; 293 of these participants were assigned to the experimental groups of knee osteoarthritis, 24 in hand osteoarthritis, 82 of low back pain, 152 with reumathoid arthritis, and 32 with osteoporosis. They were treated with mineral water baths and/or mud/peloid (with or without other forms of treatment, like physical therapy, exercise…). The rest were allocated to the control groups; they received mainly tap water and/or "non-mineral" mud/peloid treatments. Mineral water or mud treatments had better and longer improvements in pain, function, quality of life, clinical parameters, and others in some rheumatologic diseases (knee and hand osteoarthritis, chronic low back pain, rheumatoid arthritis, and osteoporosis) compared to baseline and non-mineral similar treatments. Internal validity and other limitations of the study's methodology impede causal relation of spa therapy on these improvements. Randomized clinical trials are very heterogeneous. Double-blind randomized clinical trials seem to be the key for studying the role of mineral elements and other chemical compounds, observing enough consistency to demonstrate better and longer improvements for mineral waters or derivate compared to tap water; but due to heterogeneity and gaps on study protocol and methodology, existing research is not sufficiently strong to draw firm conclusions. Well-designed studies in larger patients' population are needed to establish the role of minerals and other chemical compounds in spa therapy.

    Except for compression therapy, physical therapy has scarcely been evaluated in the treatment of chronic venous disorders (CVD). Spa treatment is a popular way to administer physical therapy for CVD in France, but its efficacy has not been evaluated yet. This study aimed to assess the efficacy of balneotherapy associated with patient education, as performed in the spa resort of La Léchère, in patients with advanced chronic venous insufficiency (CEAP clinical classes C4/C5). The study was a randomized controlled trial, spa therapy being administered on top of the usual medical care. Evaluation was by a blinded independent investigator. Subjects were patients with primary or post-thrombotic CVD with skin changes but no active ulcer (C4a, C4b, or C5), living in Grenoble area, and willing to undergo a spa treatment course in La Léchère. The treated group had the three week spa treatment course in La Léchère, soon after randomization; the control group also had a spa treatment, but starting at day 365. The treatment consisted of four balneology sessions per day, six days a week during three weeks, and three educational workshops. An independent follow-up was performed in Grenoble hospital every three months for 15 months. The main outcome criterion was the severity of the skin changes, as evaluated by means of malleolar chromametry. Quality of life, as measured by the Chronic Venous Insufficiency Questionnaire 2 scale, a visual analog scale (VAS) for leg symptoms, and the occurrence of leg ulcers were used as secondary criteria. The year after spa treatment in the treated group was compared with the year before spa treatment in the control group. Fifty-nine subjects were enrolled (29 in the treatment group and 30 in the control group). No statistically significant difference between groups was found at study onset regarding age, sex, etiology, CEAP "C" class, and the outcome variables. After treatment, chromametry showed significantly decreased pigmentation and erythema in the treatment group compared with the controls (P < .01). Quality of life (P < .01) and symptoms (P < .001) also improved significantly. These differences remained significant after one year follow-up. The control patients improved similarly after their own spa treatment (day 450). This study shows that spa therapy, associating balneotherapy and patient education, is able to improve significantly the skin trophic changes of the CVD patients and their CVD related quality of life and symptoms. This effect is of large magnitude and remains significant one year after the treatment course.

  14. Environmental problems of resort towns of Caucasian Mineralnye Vody region during the anthropocene era

    There has been studied the influence of ecological factors on rehabilitation properties of the atmospheric surface layer (ASL) in the mountain resorts of the Caucasian Mineralnye Vody Region (CMVR) according to the research methods [1] accepted in balneology. Taking into consideration the data of the long-term complex multiple-factor bioclimatic monitoring (PRIC FMBA, IFA RAS, SNP), it has been revealed: - the increase in frequency of inter-day variability of the integral index of weather pathogenicity (IIWP) in the range of 0,352,0). These signs are noted on the background of the continuing period of climate warming. At the present time the rehabilitation potential of ASL in the resorts of Caucasian Mineralnye Vody region is estimated within 2,08-2,68 points (max =3,0 points). Mountainous areas are more sensitive to anthropogenic impacts, changes of radiation mode and circulation modes. It was revealed that there are a number of phenomena and trends in ASL of the mountainous sites, which are negative markers of urbanization process and advent of Anthropocene era. Therefore, it is actual to form a new concept of sustainable development of mountainous resort towns with scientifically grounded territorial complex system of conservation based on the modern principles of resort town-planning that will contain some scientifically proven and legally defended criteria of environmental pressures, principles of environmentally protective gardening, regulation of transportation flows, ecologically reasonable methods of waste elimination. Finally, all the efforts must be directed to preserving natural medical resources and ensuring of their renewal. References: 1.   15. Specific weather biotrop factors in the mountain resorts of North Caucasus

    Key aspects of weather therapeutic action in the mountain resorts of the Northern Caucasus (RNC) are formed under the combined influence of global, regional and local atmospheric processes, picturesque landscape, vegetation which create specificity and attraction of the weather regime for the interests of resort rehabilitation, recreation and tourism practically during the whole year. They include high purity of surface atmosphere (background level of aerosols for the countryside, the transparency of the atmosphere 0.780 -0.890); natural hypo barium and hypoxia (5-10%); increased natural aeroionization (N+=400-1000 ion/cm3; N- = 600-1200 ion/cm3; KUI = 0.8 -1.0); the softness of temperature rate (± 600 W/m ); regime of solar radiation favourable for heliotherapy. Pathogenic effects in the mountains can occur both in front atmospheric processes and in conditions of relatively favorable weather. For example, in a stable anti-cyclonic air mass with the sunny weather, foehn effects can happen that are accompanied by excessively low air humidity (lower than 20-30%), the air temperature rises in the afternoon (in winter until 15- 20°C, in summer - up to 25-35°C). The situation can be worsened by ozone intrusion (O3) with the increase of its concentration by 20 ppb or more, temperature stratification change, formation of pollution accumulation conditions in the gorges and valleys where the resort towns are located. We can observe: the increase in the concentration of aerosol pollution from 1.78 to 4 and even up to 8-10 particles/cm (particle diameter is 500-1000 nm); the rise in mass concentration of submicron aerosol up to 75 mkg/m3 and the gas pollution (CO, COx, O3) of the surface atmosphere. Against this backdrop the effects of rapid changes in the chemical composition of natural ions due to the formation of positive nitrogen ions (often with a prevalence of positive over negative air ions) can be sometimes developed. In such situations people suffering from disadaptation are under the risk of expanding meteopathic reactions which require medical intervention. Long-term performance of medical weather forecast system (MWFS) has proved its high social role - the effectiveness of spa rehabilitation of people with disadaptation in RNC through planned meteoprophylaxis increases by 20-30% [1]. Unfortunately, there are still many methodological aspects of forecasting biotropic situations for balneology which are insufficiently studied in the aspect of MWFS.
