A history of the public-private partnership 'SmartTots' between the IARS and FDA is presented. In order to raise money for research to better understand the relationship between sedative and anesthetic agents and neurotoxicity in the developing brain, the FDA approached the IARS in 2008. A partnership was developed over the following 2 years, then a Scientific Advisory Board was created to develop a research agenda. The IARS contributed $200 000 in 2011 to provide initial funding; 33 proposals were submitted in response to a request for proposals in late 2011 and resulted in the awarding of two, $100 000 grants in 2012. An Executive Board was appointed under the leadership of Michael Roizen to spearhead additional fund-raising efforts, and a director of development is working with Dr. Roizen and the Board to raise funds from individuals and organizations. Dr. Roizen has personally committed to a matching grant for anesthesiologists, up to $50 000 per year for 20 years ($1 million). Readers of the journal are encouraged to go to the website www.smarttots.org in order to better understand the issue, to contribute to the research fund themselves, and to encourage their own professional organizations to partner with SmartTots in fund-raising. © 2012 Blackwell Publishing Ltd.

In August, 1999, the National Aeronautics and Space Administration issued a Cooperative Agreement (CA) to the State of Louisiana, through the Louisiana Board of Regents (BOB), for the performance of scientific research and graduate fellowships under the NASA Experimental Program to Stimulate Competitive Research (EPSCoR) -- Preparation Grant. Originally constructed as a one year program, with an optional one year continuation, this federal-state partnership culminated on 14 August 2002, including a successful continuation proposal and a no cost extension. The total value of the project reached $450K in NASA funding, matched by state funds and institutional contributions. The purpose of the Preparation Grant program was to develop and nurture strong research ties between the state and NASA field centers and Enterprises, in order to prepare for the upcoming full competition for NASA EPSCoR.

To determine if the proportion of consumers on federally qualified health center (FQHC) governing boards is associated with their use of federal grant funds to provide uncompensated care. Using FQHC data from the Uniform Data System, county-level data from the Area Resource File and governing board data from FQHC grant applications, the uncompensated care an FQHC provides relative to the amount of its federal funding is modeled as a function of board and executive committee composition using fixed-effects regression with FQHC and county-level controls. Consumer governance does not predict how much uncompensated care an FQHC provides relative to the size of its federal grant. Rather, the proportion of an FQHC's patient-mix that is uninsured drives uncompensated care provision. Aside from a small executive committee effect, consumer governance does not influence FQHCs' provision of uncompensated care. More work is needed to understand the role of consumer governance.

Legislation currently being considered by both the U.S. House and Senate would create a National Hurricane Research Initiative. The legislation was developed in response to a January 2007 U.S. National Science Board report,"Hurricane warning: The critical need for a National Hurricane Research Initiative." Both bills require the hurricane research initiative to set objectives in order to make recommendations to the National Science Board and to assemble U.S. science and engineering expertise through an interagency effort designed to bring together the latest research focusing on infrastructure, forecasting, and mitigating impacts on coastal populations. The bills also require the initiative to set objectives for making grants for hurricane research on a variety of topics, ranging from hurricane dynamics to improving emergency communications networks. Coordination of the interagency effort would fall under the jurisdiction of the White House Office of Science and Technology Policy.

The history of forensic anthropology has been documented by numerous scholars. These contributions have described the work of early pioneers in the field and have described important milestones, such as the founding of the Physical Anthropology Section of the American Academy of Forensic Sciences (AAFS) in 1972 and the American Board of Forensic Anthropology (ABFA) in 1977. This paper contributes to the growing literature on the history of forensic anthropology by documenting the academic training of all individuals who have been granted diplomate status by the ABFA (n = 115). Doctoral dissertation titles were queried to discern broad patterns of research foci. A total of 39 doctoral granting institutions have trained diplomates and 77.3% of board-certified forensic anthropologists wrote dissertations involving skeletal biology, bioarchaeology, or forensic anthropology. Board-certified forensic anthropologists are a broadly trained group of professionals with far-reaching anthropological interests and expertise. © 2016 American Academy of Forensic Sciences.

Hospital board members are asked to consider large amounts of quality and safety data with a duty to act on signals of poor performance. However, in order to do so it is necessary to distinguish signals from noise (chance). This article investigates whether data in English National Health Service (NHS) acute care hospital board papers are presented in a way that helps board members consider the role of chance in their decisions. Thirty English NHS trusts were selected at random and their board papers retrieved. Charts depicting quality and safety were identified. Categorical discriminations were then performed to document the methods used to present quality and safety data in board papers, with particular attention given to whether and how the charts depicted the role of chance, that is, by including control lines or error bars. Thirty board papers, containing a total of 1488 charts, were sampled. Only 88 (6%) of these charts depicted the role of chance, and only 17 of the 30 board papers included any charts depicting the role of chance. Of the 88 charts that attempted to represent the role of chance, 16 included error bars and 72 included control lines. Only 6 (8%) of the 72 control charts indicated where the control lines had been set (eg, 2 vs 3 SDs). Hospital board members are expected to consider large amounts of information. Control charts can help board members distinguish signals from noise, but often boards are not using them. We discuss demand-side and supply-side barriers that could be overcome to increase use of control charts in healthcare. Published by the BMJ Publishing Group Limited. A Study of Experience Credit for Professional Engineering Licensure

Oak Ridge National Laboratory performed a study of experience credit for professional engineering licensure for the Department of Energy's Industrial Assessment Center (IAC) Program. One of the study's goals was to determine how state licensure boards grant experience credit for engineering licensure, particularly in regards to IAC experience and experience prior to graduation. Another goal involved passing IAC information to state licensure boards to allow the boards to become familiar with the program and determine if they would grant credit to IAC graduates. The National Council of Examiners for Engineers and Surveyors (NCEES) has adopted a document, the ''Model Law''.more » This document empowers states to create state engineering boards and oversee engineering licensure. The board can also interpret and adopt rules and regulations. The Model Law also gives a general ''process'' for engineering licensure, the ''Model Law Engineer''. The Model Law Engineer requires that an applicant for professional licensure, or professional engineering (PE) licensure, obtain a combination of formal education and professional experience and successfully complete the fundamentals of engineering (FE) and PE exams. The Model Law states that a PE applicant must obtain four years of ''acceptable'' engineering experience after graduation to be allowed to sit for the PE exam. Although the Model Law defines ''acceptable experience,'' it is somewhat open to interpretation, and state boards decide whether applicants have accumulated the necessary amount of experience. The Model Law also allows applicants one year of credit for postgraduate degrees as well as experience credit for teaching courses in engineering. The Model Law grants states the power to adopt and amend the bylaws and rules of the Model Law licensure process. It allows state boards the freedom to modify the experience requirements for professional licensure. This power has created variety in experience requirements, and licensure requirements can differ from state to state. Before this study began, six questions were developed to help document how state boards grant experience credit. Many of the questions were formulated to determine how states deal with teaching experience, postgraduate credit, experience prior to graduation, PE and FE waivers, and the licensure process in general. Data were collected from engineering licensure boards for each of the fifty states and the District of Columbia. Telephone interviews were the primary method of data collection, while email correspondence was also used to a lesser degree. Prior to contacting each board, the researchers attempted to review each state's licensure web site. Based on the data collected, several trends and patterns were identified. For example, there is a general trend away from offering credit for experience prior to graduation. The issue becomes a problem when a PE from one state attempts to gain a license in another state by comity or endorsement. Tennessee and Kansas have recently stopped offering this credit and Mississippi cautions applicants that it could be difficult to obtain licensure in other states.« less

Purpose: Intensity modulated radiotherapy requires a comprehensive quality assurance program in general and ideally independent verification of dose delivery. Since conventional 2D detector arrays allow only pre-treatment verification, there is a debate concerning the need of online dose verification. This study presents the clinical performance, including dosimetric plan verification in 2D as well as in 3D and the error detection abilities of a new transmission detector (TD) for online dose verification of 6MV photon beam. Methods: To validate the dosimetric performance of the new device, dose reconstruction based on TD measurements were compared to a conventional pre-treatment verification method (reference)more » and treatment planning system (TPS) for 18 IMRT and VMAT treatment plans. Furthermore, dose reconstruction inside the patient based on TD read-out was evaluated by comparing various dose volume indices and 3D gamma evaluations against independent dose computation and TPS. To investigate the sensitivity of the new device, different types of systematic and random errors for leaf positions and linac output were introduced in IMRT treatment sequences. Results: The 2D gamma index evaluation of transmission detector based dose reconstruction showed an excellent agreement for all IMRT and VMAT plans compared to reference measurements (99.3±1.2)% and TPS (99.1±0.7)%. Good agreement was also obtained for 3D dose reconstruction based on TD read-out compared to dose computation (mean gamma value of PTV = 0.27±0.04). Only a minimal dose underestimation within the target volume was observed when analyzing DVH indices (<1%). Positional errors in leaf banks larger than 1mm and errors in linac output larger than 2% could clearly identified with the TD. Conclusion: Since 2D and 3D evaluations for all IMRT and VMAT treatment plans were in excellent agreement with reference measurements and dose computation, the new TD is suitable to qualify for routine treatment plan verification. The American Board of Orthodontics was established in 1929 and is the oldest specialty board in dentistry. Its goal is to protect the public by ensuring competency through the certification of eligible orthodontists. Originally, applicants for certification submitted a thesis, 5 case reports, and a set of casts with appliances. Once granted, the certification never expired. Requirements have changed over the years. In 1950, 15 cases were required, and then 10 in 1987. The Board has continued to refine and improve the certification process. The grant in question, DE-FG03-94ER75954, was awarded to Insights El Paso Science Museum to build key exhibits. These exhibits helped the Museum fulfill its mission to ``promote curiosity and stimulate interest by exploratory, entertaining, exciting, and participatory learning in a broad range of scientific disciplines to persons of all ages regionally and internationally.`` There are several current Board of Directors members who also were Board members during the grant period and who helped construct some of the exhibits. Based on the Wronskian technique and Lax pair, double Wronskian solution of the nonisospectral BKP equation is presented explicitly. The source counts or the so-called log N -- log S relations are the primary data that constrain the spatial distribution of sources with unknown distances, such as gamma-ray bursts. In order to test galactic, halo, and cosmological models for gamma-ray bursts we compare theoretical characteristics of the log N -- log S relations to those obtained from data gathered by the BATSE instrument on board the Compton Observatory (GRO) and other instruments. We use a new and statistically correct method, that takes proper account of the variable nature of the triggering threshold, to analyze the data. Constraints on models obtained by this comparison will be presented. This work is supported by NASA grants NAGW 2290, NAG5 2036, and NAG5 1578.

If at first you don't get funded, try, try, try again. Match your program with another funder. Some funders, such as private foundations, have ongoing deadlines. Grant funding is extremely competitive and can be political. Network with funders. Also network with board members, businesses, etc., who may know someone who serves on the grant funders review committee. Take a grant writing workshop and have your proposal reviewed by someone who can provide feedback. Gather grant-writing resources in the form of books, workshop materials, and Websites. Lastly, try to keep abreast of the trends in the grant field. The latest trend I have heard is an interest in having the problem statement become more positive. Focus on the strengths upon which you can build rather than the weaknesses of a community. Grant money is available from many resources in differing amounts. Test your ability at a local level. Begin developing a track record, then work to the state and federal levels. It is an exciting and challenging opportunity to better your community.

This article provides a description of a Community/University Collaborative Board, a formalized partnership between representatives from an inner-city community and university-based researchers. This Collaborative Board oversees a number of research projects focused on designing, delivering and testing family-based HIV prevention and mental health focused programs to elementary and junior high school age youth and their families. The Collaborative Board consists of urban parents, school staff members, representatives from community-based agencies and university-based researchers. One research project, the CHAMP (Collaborative HIV prevention and Adolescent Mental health Project) Family Program Study, an urban, family-based HIV prevention project will be used to illustrate how the Collaborative Board oversees a community-based research study. The process of establishing a Collaborative Board, recruiting members and developing subcommittees is described within this article. Examples of specific issues addressed by the Collaborative Board within its subcommittees, Implementation, Finance, Welcome, Research, Grant writing, Curriculum, and Leadership, are detailed in this article along with lessons learned.

This program began as a grant from NASA Headquarters, NGT-10025, which was in effect from 10/l/93 until 10/31/96. The remaining funding for this effort was transferred from NASA Headquarters to NASA Langley and a new grant NGT-1-52155 was issued covering the period II/l/96 to 5/15/99. This report serves as the final report of NGT-1-52155. For a number of years, Virginia Tech had been on the forefront of research in the area of multidisciplinary analysis and design. In June of 1994, faculty members from aerospace and ocean engineering, engineering science and mechanics, mechanical engineering, industrial engineering, mathematics and computer sciences, at Virginia Tech joined together to form the Multidisciplinary Analysis and Design (MAD) Center for Advanced Vehicles. The center was established with the single goal: to perform research that is relevant to the needs of the US industry and to foster collaboration between the university, government and industry. In October of 1994, the center was chosen by NASA headquarters as one of the five university centers to establish a fellowship program to develop a graduate program in multidisciplinary analysis and design. The fellowship program provides full stipend and tuition support for seven U. S. students per year during their graduate studies. The grant is currently being administered by the NMO Branch of NASA Langley. To advise us regarding the problems faced by the industry, an industrial advisory board has been formed consisting of representatives from industry as well as government laboratories. The present membership includes major aerospace companies: Aurora Flight Sciences, Boeing: Philadelphia, Boeing: Long Beach, Boeing: Seattle, Boeing: St. Louis, Cessna, Ford, General Electric, Hughes, Lockheed-Martin: Palo Alto, Northrop-Grumman, Sikorsky, smaller, aerospace software companies: Aerosoft, Phoenix Integration and Proteus Engineering, along with representatives from government agencies, including: NASA Ames, Langley and Lewis. The function of the advisory board is to channel information from its member companies to faculty members concerning problems that need research attention in the general area of multidisciplinary design optimization (MDO). The faculty and their graduate students make presentations to the board on their research. The board makes recommendations on the research and suggests new areas and problems which need attention. Many students participating in the program spend 3-6 months in industry working on their research projects. We are completing the fifth year of the fellowship program and have had four advisory board meetings in Blacksburg. Ten students have spent the three month periods in industry. In addition to the research element of the MAD Center efforts we also have an academic component. We have developed a menu of design-related graduate courses and two new courses: one in Aerospace Manufacturing and another in MDO. Time needed to board an airplane: a power law and the structure behind it.

A simple model for the boarding of an airplane is studied. Passengers have reserved seats but enter the airplane in arbitrary order. Queues are formed along the aisle, as some passengers have to wait to reach the seats for which they have reservation. We label a passenger by the number of his or her reserved seat. In most cases the boarding process is much slower than for the optimal situation, where passenger and seat orders are identical. We study this dynamical system by calculating the average boarding time when all permutations of N passengers are given equal weight. To first order, the boarding time for a given permutation (ordering) of the passengers is given by the number s of sequences of monotonically increasing values in the permutation. We show that the distribution of s is symmetric on [1,N], which leads to an average boarding time (N+1)/2. We have found an exact expression for s and have shown that the full distribution of s approaches a normal distribution as N increases. However, there are significant corrections to the first-order results, due to certain correlations between passenger ordering and the substrate (seat ordering). This occurs for some cases in which the sequence of the seats is partially mirrored in the passenger ordering. These cases with correlations have a boarding time that is lower than predicted by the first-order results. The large number of cases with reduced boarding times have been classified. We also give some indicative results on the geometry of the correlations, with sorting into geometry groups. With increasing N, both the number of correlation types and the number of cases belonging to each type increase rapidly. Using enumeration we find that as a result of these correlations the average boarding time behaves like N(α), with α≃0.69, as compared with α=1.0 for the first-order approximation. © 2012 American Physical Society

NASA Astrophysics Division supports a pre-college teacher program to provide Flight Opportunities for Science Teacher EnRichment (FOSTER). To date, forty-five teachers are participating, and the program will expand nation-wide to serve fifty teachers per year on board the Kuiper Airborne Observatory. In the future, the Stratospheric Observatory for Infrared Astronomy (SOFIA) will bring more than one-hundred teachers per year on board for astronomical research mission. FOSTER is supported by a grant to the SETI Institute from the NASA Astrophysics Division, NAGW-3291.

Small Satellites for Secondary Students is a program funded by a three-year grant from NASA to bridge the gap in STEM education for secondary-school students. This is accomplished by creating the educational resources that are needed to support the development of a small scientific payload in alignment with scientific and technological education standards. The prototype payloads are flexible multi-experiment platforms designed to accommodate a wide range of student abilities with minimal resource requirements. The heart of each payload is an Arduino microcontroller which communicates with components that provide sensor data, Global Positioning System information, and which offer on-board data storage. The payload is built with off-the-shelf components and a pre-etched, custom-designed connector board. The platform also supports real-time telemetry updates through the use of Wi-Fi. To date, the prototype payloads have been tested on both high-powered rockets reaching over 3km and weather balloons tethered at 300m. Multiple successful rocket test runs reaching 1km have been conducted in partnership with amateur rocket clubs including the Association of Experimental Rocketry of the Pacific. From these flights, we are continuing to improve the payload design in order to increase the likelihood of student success.

The governing board's role in health care is not changing, but new competitive forces necessitate a refinement of the board's approach to fulfilling its role. In a free-standing, community, not-for-profit hospital, the board functions as though it were the "owner." Although it does not truly own the facility in the legal sense, the board does have legal, fiduciary, and financial responsibilities conferred on it by the state. In a religious-sponsored facility, the board fulfills these same obligations on behalf of the sponsoring institute, subject to the institute's reserved powers. In multi-institutional systems, the hospital board's power and authority depend on the role granted it by the system. Boards in all types of facilities are currently faced with the following challenges: Fulfilling their basic responsibilities, such as legal requirements, financial duties, and obligations for the quality of care. Encouraging management and the board itself to "think strategically" in attacking new competitive market forces while protecting the organization's traditional mission and values. Assessing recommended strategies in light of consequences if constituencies think the organization is abandoning its commitments. Boards can take several steps to match their mode of operation with the challenges of the new environment. Boards must rededicate themselves to the hospital's mission. Trustees must expand their understanding of health care trends and issues and their effect on the organization. Boards must evaluate and help strengthen management's performance, rather than acting as a "watchdog" in an adversarial position. In modern nanotechnology, two-dimensional atomic network structures boast promising applications as nanoscale circuit boards to serve as the building blocks of more sustainable and efficient, electronic devices. However, properties associated with the network connectivity can be beneficial or deleterious to the current flow. Taking a computational approach, we will study large uniform networks, as well as large random networks using Kirchhoff's Equations in conjunction with graph theoretical measures of network connectedness and flows, to understand how network connectivity affects overall ability for successful current flow throughout a network. By understanding how connectedness affects flow, we may develop new ways to design more efficient two-dimensional materials for the next generation of nanoscale electronic devices, and we will gain a deeper insight into the intricate balance between order and chaos in the universe. Criminal proceedings against physicians involving an alleged professional malpractice usually represent a complex problem raising issues in various (potential) areas of conflict. In many cases such proceedings are accompanied by liability actions in civil courts, occasionally proceedings have been initiated by the Medical Board in order to clarify a case of professional misconduct. Often, a medical malpractice insurance is involved. In order to protect the physician's interests in the best possible way, any action in the various matters needs concerted efforts. No statement or comment regarding the allegations should be rendered to the prosecution before access to the investigation file has been granted. No negative conclusions may be drawn from the use of the right to refuse testimony. The preparation of a written statement requires a thorough legal and factual analysis of the allegations as well as the evidence they are based on. In this respect professional legal assistance is essential. It does not constitute a breach of the doctor-patient confidentiality if the physician discloses information in order to defend himself against criminal allegations. If several physicians are charged with an offense it is recommendable in most cases to coordinate defense activities.

One injury mechanism in ice hockey is impact with the boards. We investigated whether more flexible hockey boards would provide less biomechanical loading on impact than did existing (reference) boards. We conducted impact tests with a dynamic pendulum (mass 60 kg) and with crash test dummies (ES-2 dummy, 4.76 m/s impact speed). Outcomes were biomechanical loading experienced by a player in terms of head acceleration, impact force to the shoulder, spine, abdomen and pelvis as well as compression of the thorax. The more flexible board designs featured substantial displacement at impact. Some so-called flexible boards were displaced four times more than the reference board. The new boards possessed less stiffness and up to 90 kg less effective mass, reducing the portion of the board mass a player experienced on impact, compared with boards with a conventional design. Flexible boards resulted in a similar or reduced loading for all body regions, apart from the shoulder. The displacement of a board system did not correlate directly with the biomechanical loading. Flexible board systems can reduce the loading of a player on impact. However, we found no correlation between the displacement and the biomechanical loading; accordingly, displacement alone was insufficient to characterise the overall loading of a player and thus the risk of injury associated with board impact. The work on the grant was mainly directed towards developing new, accurate, redundant methods for the in-flight, absolute radiometric calibration of satellite multispectral imaging systems and refining the accuracy of methods already in use. Initially the work was in preparation for the calibration of MODIS and HIRIS (before the development of that sensor was canceled), with the realization it would be applicable to most imaging multi- or hyper-spectral sensors provided their spatial or spectral resolutions were not too coarse. The work on the grant involved three different ground-based, in-flight calibration methods reflectance-based radiance-based and diffuse-to-global irradiance ratio used with the reflectance-based method. This continuing research had the dual advantage of: (1) developing several independent methods to create the redundancy that is essential for the identification and hopefully the elimination of systematic errors; and (2) refining the measurement techniques and algorithms that can be used not only for improving calibration accuracy but also for the reverse process of retrieving ground reflectances from calibrated remote-sensing data. The grant also provided the support necessary for us to embark on other projects such as the ratioing radiometer approach to on-board calibration (this has been further developed by SBRS as the 'solar diffuser stability monitor' and is incorporated into the most important on-board calibration system for MODIS)- another example of the work, which was a spin-off from the grant funding, was a study of solar diffuser materials. The introduction of anti-vascular endothelial growth factor (anti-VEGF) drugs to ophthalmology has revolutionized the treatment of neovascular age-related macular degeneration (nAMD). Despite this significant progress, gaps and challenges persist in the diagnosis of nAMD, initiation of treatment, and management of frequent intravitreal injections. Thus, nAMD remains a leading cause of blindness in the United States. To present current knowledge, evidence, and expert perspectives on anti-VEGF therapies in nAMD to support managed care professionals and providers in decision making and collaborative strategies to overcome barriers to optimize anti-VEGF treatment outcomes among nAMD patients. Three anti-VEGF therapies currently form the mainstay of treatment for nAMD, including 2 therapies approved by the FDA for treatment of nAMD (aflibercept and ranibizumab) and 1 therapy approved by the FDA for oncology indications and used off-label for treatment of nAMD (bevacizumab). In clinical trials, each of the 3 agents maintained visual acuity (VA) in approximately 90% or more of nAMD patients over 2 years. However, in long-term and real-world settings, significant gaps and challenges in diagnosis, treatment, and management pose barriers to achieving optimal outcomes for patients with nAMD. Many considerations, including individual patient characteristics, on-label versus off-label treatment, repackaging, and financial considerations, add to the complexity of nAMD decision making and management. Many factors may contribute to additional challenges leading to suboptimal long-term outcomes among nAMD patients, such as delays in diagnosis and/or treatment approval and initiation, individual patient response to different anti-VEGF therapies, lapses in physician regimentation of anti-VEGF injection and monitoring, and inadequate patient adherence to treatment and monitoring. These latter factors highlight the considerable logistical, emotional, and financial burdens of long-term, frequent intravitreal injections and the vital importance of personalized approaches to anti-VEGF treatment decision making and management for patients with nAMD. To address these challenges and reduce the number of yearly injections, studies have examined alternative dosing regimens, including extended fixed intervals, as needed, and treat-and-extend strategies in specific nAMD patient populations. New clinical evidence and insights into expert clinical practice discussed in this article can support managed care professionals in the key role they play in addressing challenges in nAMD treatment and management and optimizing patient outcomes through appropriate management of anti-VEGF treatment. The system consists of one main printed circuit board (PCB) and 16 channel boards (each channel board contains 8 channels), in addition to a set-top box for connections to a personal computer (PC), a front panel board for user control and message display, and a power control and distribution board. The main board contains a field programmable gate array (FPGA) and controls all channels (each channel has also an FPGA). We will report the analog and digital circuit design and simulations, multiplayer PCB designs with commercial software (Protel 99), PCB signal integrity testing and system RFI/EMI shielding, and the assembly and construction of the entire system. What is an objective approach that employs measurable and reproducible physiologic changes as the basis for the classification of ovarian hyperstimulation syndrome (OHSS) in order to facilitate more accurate reporting of incidence rates within and across clinical trials? The OHSS flow diagram is an objective approach that will facilitate consistent capture, classification and reporting of OHSS within and across clinical trials. OHSS is a potentially life-threatening iatrogenic complication of the early luteal phase and/or early pregnancy after ovulation induction (OI) or ovarian stimulation (OS). The clinical picture of OHSS (the constellation of symptoms associated with each stage of the disease) is highly variable, hampering its appropriate classification in clinical trials. Although some degree of ovarian hyperstimulation is normal after stimulation, the point at which symptoms transition from those anticipated to those of a disease state is nebulous. An OHSS working group, comprised of subject matter experts and clinical researchers who have significantly contributed to the field of fertility, was convened in April and November 2014. The OHSS working group was tasked with reaching a consensus on the definition and the classification of OHSS for reporting in clinical trials. The group engaged in targeted discussion regarding the scientific background of OHSS, the criteria proposed for the definition and the rationale for universal adoption. An agreement was reached after discussion with all members. One of the following conditions must be met prior to making the diagnosis of OHSS in the context of a clinical trial: (i) the subject has undergone OS (either controlled OS or OI) AND has received a trigger shot for final oocyte maturation (e.g. hCG, GnRH agonist [GnRHa] or kisspeptin) followed by either fresh transfer or segmentation (cryopreservation of embryos) or (ii) the subject has undergone OS or OI AND has a positive pregnancy test. All study patients who develop symptoms of OHSS should undergo a thorough examination. An OHSS flow diagram was designed to be implemented for all subjects with pelvic or abdominal complaints, such as lower abdominal discomfort or distention, nausea, vomiting and diarrhea, and/or for subjects suspected of having OHSS. The diagnosis of OHSS should be based on the flow diagram. This classification system is primarily intended to address the needs of the clinical investigator undertaking clinical trials in the field of OS and may not be applicable for the use in clinical practice or with OHSS occurring under natural circumstances. The proposed OHSS classification system will enable an accurate estimate of the incidence and severity of OHSS within and across clinical trials performed in women with infertility.
