To describe the American Board of Psychiatry and Neurology (ABPN) Maintenance of Certification Program, its underlying rationale, how it will be implemented now, and what it might look like in the future. The authors describe the philosophical foundation, specific components, and the implementation timeline of the ABPN Maintenance of Certification Program; the development of specific products that might be used by ABPN diplomates to meet its requirements; and several unanswered questions about its current status and future development. The ABPN Maintenance of Certification Program consists of specific requirements pertaining to professional standing, self-assessment and lifelong learning, performance in practice, and cognitive expertise that will be implemented incrementally over the next decade. The ABPN Maintenance of Certification Program has been implemented in a manner that is as consistent as possible with its underlying philosophical beliefs as well as the current and expected public and political concerns, diplomate needs, and the requirements of organizations responsible for licensure, credentialing, privileging, accreditation, professional development, and physician reimbursement.

Part 65, Subparts D and E, of the Federal Aviation Regulations (FAR) identify the certification requirements for aviation mechanics and aviation repairmen. The training, experience, privileges, ratings, recordkeeping, and currency requirements for aviation maintenance personnel are also addressed by those parts of the FAR. The recent emergence of the aging fleet problem and the introduction of new technologies, aircraft, engines, and aeronautical products has caused certain portions of these rules to become obsolete. Further, international political arrangements, such as bilateral airworthiness and maintenance agreements, International Civil Aviation Organization (ICAO) standards, certain international agreements for maintenance personnel training, and mechanic certificate reciprocity, have all impacted on the current regulatory policy.

ABR MOC has been developed in accord with guidelines of the American Board of Medical Specialties. All Radiation Oncology certificates issued since 1995 are 10-year, time-limited certificates; diplomates with time-limited certificates who wish to maintain specialty certification must complete specific requirements ofmore » the American Board of Radiology MOC program. Diplomates with lifelong certificates are not required to participate but are strongly encouraged to do so. Maintenance of Certification is based on documentation of participation in the four components of MOC: (1) professional standing, (2) lifelong learning and self-assessment, (3) cognitive expertise, and (4) performance in practice. Through these components, MOC addresses six competencies-medical knowledge, patient care, interpersonal and communication skills, professionalism, practice-based learning and improvement, and systems-based practice. Establishing and managing a board certification system is a common concern for many countries. In Japan, the board certification system is under revision. The purpose of this study was to describe present status of internal medicine specialist board certification, to identify factors associated with maintenance of board certification and to investigate changes in area of practice when physicians move from hospital to clinic practice. We analyzed 2010 and 2012 data from the Survey of Physicians, Dentists and Pharmacists. We conducted logistic regression analysis to identify factors associated with the maintenance of board certification between 2010 and 2012. We also analyzed data on career transition from hospitals to clinics for hospital physicians with board certification. It was common for physicians seeking board certification to do so in their early career. The odds of maintaining board certification were lower in women and those working in locations other than academic hospitals, and higher in physicians with subspecialty practice areas. Among hospital physicians with board certification who moved to clinics between 2010 and 2012, 95.8% remained in internal medicine or its subspecialty areas and 87.7% maintained board certification but changed their practice from a subspecialty area to more general internal medicine. Revisions of the internal medicine board certification system must consider different physician career pathways including mid-career moves while maintaining certification quality. This will help to secure an adequate number and distribution of specialists. To meet the increasing demand for generalist physicians, it is important to design programs to train specialists in general practice.

Continuing medical education serves a central role in the licensure and certification for practicing physicians. This chapter explores the different modalities that constitute CME along with their effectiveness, including simulation and best education practices. The evolution to maintenance of certification and the requirements for both the American Board of Surgery and the American Board of Colon and Rectal Surgery are delineated. Further progress in the education of practicing surgeons is evidenced through the introduction of laparoscopic colectomy and the improvements made from the introduction of laparoscopic cholecystectomy. Finally, reentry of physicians into practice following a voluntary leave of absence, a new and challenging issue for surgeons, is also discussed. PMID:23997673

An Effectiveness Trial of Contingency Management in a Felony Preadjudication Drug Court

This study evaluated a contingency management (CM) program in a drug court. Gift certificates for compliance were delivered at 4- to 6-week intervals (total value  =  $390.00). Participants in one condition earned gift certificates that escalated by $5.00 increments. Participants in a second condition began earning higher magnitude gift certificates, and the density of reinforcement was gradually decreased. No main effects of CM were detected, which appears to be attributable to a ceiling effect from the intensive contingencies already delivered in the drug court and the low density of reinforcement. Preplanned interaction analyses suggested that participants with more serious criminal backgrounds might have performed better in the CM conditions. This suggests that CM programs may be best suited for more incorrigible drug offenders. PMID:19192860

Maintenance of Certification (MOC) recognizes that in addition to medical knowledge, several essential elements involved in delivering quality care must be developed and maintained throughout one's career. The MOC process is designed to facilitate and document the professional development of each diplomate of The American Board of Radiology (ABR) through its focus on the essential elements of quality care in Diagnostic Radiology and its subspecialties, and in the specialties of Radiation Oncology and Radiologic Physics. The initial elements of the ABR-MOC have been developed in accord with guidelines of The American Board of Medical Specialties. All diplomates with a ten-year,more » time-limited primary certificate in Diagnostic Radiologic Physics, Therapeutic Radiologic Physics, or Medical Nuclear Physics who wish to maintain certification must successfully complete the requirements of the appropriate ABR-MOC program for their specialty. Holders of multiple certificates must meet ABR-MOC requirements specific to the certificates held. Diplomates with lifelong certificates are not required to participate in the MOC, but are strongly encouraged to do so. A 9-year comparison of practice profiles of candidates for primary and recertification examinations of the American Board of Plastic Surgery relative to economic indicators.

This study compared the practice profiles of plastic surgeons applying for recertification/maintenance of certification with those applying for primary certification by the American Board of Plastic Surgery between 2003 and 2011. American Board of Plastic Surgery case logs from both recertification and primary certification candidates from 2003 to 2011 were examined. Deidentified data included operative year, Current Procedural Terminology codes, and the candidate's designation of the case relative to (1) cosmetic or reconstructive and (2) the Maintenance of Certification in Plastic Surgery module (i.e., comprehensive, cosmetic, craniomaxillofacial, and hand). Department of Commerce unemployment data from 2003 to 2011 served as an economic indicator for the period studied. A negative trend in the median number of cases per candidate was observed for both groups for cosmetic, reconstructive, and total number of cases, corresponding to a rise in unemployment. With every 1 percent increase in the unemployment rate, recertification candidates demonstrated a greater loss of cosmetic cases relative to primary candidates and an accelerated decline in reconstructive cases starting in 2007. Distribution of the four Maintenance of Certification modules demonstrated a negative trend for cosmetic and comprehensive cases in both groups. Hand and craniofacial consistently constituted approximately 20 percent of cases for primary and 14 percent of cases for recertification candidates. There was a shift away from hand cases toward craniofacial cases in both groups. Both primary and recertification candidates reported a decline in overall caseload from 2003 to 2011. Negative economic trends have a greater impact on the practice profile of recertification candidates.

The Maintenance of Certification® (MOC) program has been well received by many physicians, but faced significant opposition from others, who complain that it is overly tedious, costly, and irrelevant to their practice. This article offers a consolidated and concise history of the MOC program and a summary of what plastic surgeons need to know to successfully complete the American Board of Plastic Surgery’s (ABPS) own MOC requirements. The authors have justified each step of the ABPS’s MOC process in terms of how it improves the quality of care delivered to Plastic Surgery patients. Finally, a summary of research is presented that demonstrates both that the public supports the MOC process for all physicians and that continuing education and formal assessment and improvement initiatives have been linked in multiple studies to a better and more evidence-based medical practice. Relationship Between Physicians' Active Participation in Maintenance of Certification and Patients' Perspective of Care Surveys.

Medical specialty boards have a Maintenance of Certification (MOC) paradigm whose intention is to ensure high-quality patient care. How the patient experience is affected by physician MOC enrollment/participation is unknown. Our goal was to determine if patient experience is associated with physician board certification and MOC status. We analyzed physician experience and MOC databases to determine the relationships among physicians' patient experience national percentile rankings and board certification status and MOC enrollment and activity status. Board-certified physicians enrolled in MOC did not have statistically significant different patient experience scores compared to board-certified physicians not enrolled in MOC. Mid-career physicians enrolled in MOC had patients more likely to recommend them and reported higher confidence in them. Patients did not perceive physicians participating in MOC patient safety modules as more cautious in providing patient care. Although most analyses did not demonstrate significant differences in patient experience scores for physicians actively participating in MOC compared to those not, some differences were noted. Higher provider-specific patient experience scores were noted, particularly for mid-career physicians.

Biological safety cabinets (BSCs) are the primary means of containment used in laboratories worldwide for the safe handling of infectious microorganisms. They provide protection to the laboratory worker and the surrounding environment from pathogens. To ensure the correct functioning of BSCs, they need to be properly maintained beyond the daily care routines of the laboratory. This involves annual maintenance and certification by a qualified technician in accordance to the NSF/American National Standards Institute 49-2014 Biosafety Cabinetry: Design, Construction, Performance, and Field Certification. Service programs can be direct from the manufacturer or through third-party service companies, but in many instances, technicians are not accredited by international bodies, and these services are expensive. This means that a large number of BSCs may not be operating in a safe manner. In this article, we discuss our approach to addressing the lack of trained and qualified personnel in Thailand who can install, maintain, and certify BSCs in a cost-effective and practical manner. We initiated a program to create both local and regional capacity for repair, maintenance, and certification of BSCs and share our experiences with the reader. PMID:27721674

Introduction In 2012, the Louisiana (LA) Department of Health and Hospitals revised the LA birth certificate to include medical reasons for births before 39 completed weeks' gestation. We compared the completeness and validity of these data with hospital discharge records. Methods For births occurring 4/1/2012-9/30/2012 at Woman's Hospital of Baton Rouge, we linked maternal delivery and newborn birth data collected through the National Perinatal Information Center with LA birth certificates. Among early term births (37-38 completed weeks' gestation), we quantified the reasons for early delivery listed on the birth certificate and compared them with ICD-9-CM codes from Woman's discharge data. Results Among 4353 birth certificates indicating delivery at Woman's Hospital, we matched 99.8% to corresponding Woman's administrative data. Among 1293 early term singleton births, the most common reasons for early delivery listed on the birth certificate were spontaneous active labor (57.5%), gestational hypertensive disorders (15.3%), gestational diabetes (8.7%), and premature rupture of membranes (8.1%). Only 2.7% of births indicated "other reason" as the only reason for early delivery. Most reasons for early delivery had >80% correspondence with ICD-9-CM codes. Lower correspondence (35 and 72%, respectively) was observed for premature rupture of membranes and abnormal heart rate or fetal distress. Discussion There was near-perfect ability to match LA birth certificates with Woman's Hospital records, and the agreement between reasons for early delivery on the birth certificate and ICD-9-CM codes was high. A benchmark of 2.7% can be used as an attainable frequency of "other reason" for early delivery reported by hospitals. Louisiana implemented an effective mechanism to identify and explain early deliveries using vital records.

The American Board of Medical Specialties board certification has transformed into a career-long process of learning, assessment, and performance improvement through its Program for Maintenance of Certification (MOC). Medical educators across many medical professional organizations, specialty societies, and other institutions have played important roles in shaping MOC and tailoring its overarching framework to the needs of different specialties. This Commentary addresses potential barriers to engagement in work related to MOC for medical school (MS) and academic health center (AHC) educators and identifies reasons for, and ways to accomplish, greater involvement in this work. The authors present ways that medical and other health professions educators in these settings can contribute to the continuous improvement of the MOC program including developing educational and assessment activities, engaging in debate about MOC, linking MOC with institutional quality improvement activities, and pursuing MOC-related scholarship. MS- and AHC-based educators have much to offer this still-young and continually improving program, and their engagement is sought, necessary, and welcomed.

According to the framework decision of the European Union Council, genetic laboratories which perform tests for the benefit of the law enforcement agencies and the administration of justice are required to obtain a certificate of accreditation testifying to compliance with the PN EN ISO/IEC 17025:2005 standard. The certificate is the official confirmation of the competence to perform research, an acknowledgement of credibility, impartiality and professional independence. It is also the proof of establishment, implementation and maintenance of an appropriate management system. The article presents the legal basis for accreditation, the procedure of obtaining the certificate of accreditation and selected elements of the management system.

Maintenance of Certification Part IV Quality-Improvement Project for Hypertension Control: A Preliminary Retrospective Analysis

Context: A Maintenance of Certification Part IV project was created on the basis of an existing, multifaceted hypertension improvement program. Objective: To evaluate the impact of the Maintenance of Certification project, the effects of the improvement options on blood pressure control in hypertensive patients, and the participants’ perception of the workload related to participation in the project. Design: Nonexperimental retrospective analysis. Setting: Kaiser Permanente hospitals and medical office buildings in Northern California. Intervention: Participants used one or more options from a defined menu of strategies to attempt to increase the percentage of hypertensive patients on their patient panels who had controlled blood pressure. Main Outcome Measure: Proportion of hypertensive patients with blood pressure ≤ 139/89 mm Hg. Results: Fifty-two American Board of Family Medicine and 19 American Board of Internal Medicine certified physicians completed projects. Mean panel blood pressure control improved from 79.49% (standard deviation [SD] = 11.32) to 84.64% (SD = 7.80). The choice of improvement option was not associated with the level of improvement or with the participants’ perception of the workload related to completing the project. Conclusion: Project participants improved the care of their patients without an increased perceived burden to their practice. We found no association between the choice of improvement option and either the level of improvement or the perception of workload. PMID:25785642

Since 2006, after completing a cognitive certifying or recertifying examination, dermatologists are automatically enrolled into Maintenance of Certification (MOC) and can access a personalized electronic table (at www.abderm.org) that presents the requirements over the 10-year cycle. On this web site, diplomates can also pay the annual fee and attest to completion of the various components. Clicking on hyperlinks in the table launches explanations of the requirements. this article reviews the current status and recent trends in the American Board of Psychiatry and Neurology (ABPN) psychiatric subspecialties and discusses the implications of those trends as well as several key questions whose answers may well determine subspecialty viability. data are presented on specialty and subspecialty programs; graduates; and ABPN certification candidates and diplomates drawn from several sources, including the records of the ABPN, the websites of the Accreditation Council for Graduate Medical Education and the American Medical Association, and the annual medical education issues of JAMA. fewer than half of psychiatry graduates pursue subspecialty training. While most recent specialty graduates attempt to become certified by the ABPN, many subspecialists elect not to do so. There have been recent decreases in the number of fellowship programs and trainees in geriatric psychiatry and addiction psychiatry. The pass rates for fellowship graduates are superior to those for the "grandfathers" in all of the newer psychiatric subspecialties. Lower percentages of subspecialists than specialists participate in maintenance of certification, and maintenance of certification pass rates are high. the initial interest in training and certification in some of the ABPN subspecialties appears to have slowed, and the long-term viability of those subspecialties may well depend on the answers to a number of complicated social, economic, and political questions in the new health care era.

Methods: Data on numbers of residency and fellowship programs and graduates and ABPN certification candidates and diplomates were drawn from several sources, including ABPN records, Web sites of the Accreditation Council for Graduate Medical Education and the American Medical Association, and the annual medical education issues of the Journal of the American Medical Association. Results: About four-fifths of neurology graduates pursue fellowship training. While most recent neurology and child neurology graduates attempt to become certified by the ABPN, many clinical neurophysiologists elect not to do so. There appears to have been little interest in establishing fellowships in neurodevelopmental disabilities. The pass rate for fellowship graduates is equivalent to that for the “grandfathers” in clinical neurophysiology. Lower percentages of clinical neurophysiologists than specialists participate in maintenance of certification, and maintenance of certification pass rates are high. Conclusion: The initial enthusiastic interest in training and certification in some of the ABPN neurologic subspecialties appears to have slowed, and the long-term viability of those subspecialties will depend upon the answers to a number of complicated social, economic, and political questions in the new health care era. A Study to Estimate the Effectiveness of Visual Testing Training for Aviation Maintenance Management

The Air Commerce Act of 1926 set the beginning for standards in aviation maintenance. Even after deregulation in the late l970s, maintenance standards and requirements still have not changed far from their initial criteria. After a potential candidate completes Federal Aviation Administration training prerequisites, they may test for their Airframe and Powerplant (A&P) certificate. Performing maintenance in the aviation industry for a minimum of three years, the technician may then test for their Inspection Authorization (IA). After receiving their Airframe and Powerplant certificate, a technician is said to have a license to perform. At no time within the three years to eligibility for Inspection Authorization are they required to attend higher-level inspection training. What a technician learns in the aviation maintenance industry is handed down from a seasoned technician to the new hire or is developed from lessons learned on the job. Only in Europe has the Joint Aviation Authorities (JAA) required higher-level training for their aviation maintenance technicians in order to control maintenance related accidents (Lu, 2005). Throughout the 1990s both the General Accounting Office (GAO) and the National Transportation Safety Board (NTSB) made public that the FAA is historically understaffed (GAO, 1996). In a safety recommendation the NTSB stated "The Safety Board continues to lack confidence in the FAA's commitment to provide effective quality assurance and safety oversight of the ATC system (NTSB, 1990)." The Federal Aviation Administration (FAA) has been known to be proactive in creating safer skies. With such reports you would suspect the FAA to also be proactive in developing more stringent inspection training for aviation maintenance technicians. The purpose of this study is to estimate the effectiveness of higher-level inspection training, such as Visual Testing (VT) for aviation maintenance technicians, to improve the safety of aircraft and to make recommendations to management with regard to the value of such training.

Feasibility of patient and peer surveys for Maintenance of Certification among diplomates of the American Board of Anesthesiology.

The initial developmental standards for Maintenance of Certification programs proposed by the American Board of Medical Specialties included the administration of patient and peer surveys by the diplomate every 5 years. The aim of this pilot study was to determine the feasibility of Maintenance of Certification in Anesthesiology Program (MOCA) patient and peer surveys in a selected group of American Board of Anesthesiology (ABA) diplomates. The design was a pilot test of survey instruments-MOCA Patient Care Survey and MOCA Peer Survey. The setting was the ABA, Raleigh, NC. The subjects were ABA-certified anesthesiologists who were active examiners for the primary certification oral examination as of January 2013. Fifty-one participating physicians in the patient survey group distributed brochures, which included a link to the MOCA Patient Care Survey, to up to 100 consecutive patients at the point of care. Fifty-one participating physicians in the peer survey group distributed invitations to MOCA Peer Survey via e-mail to 20 peers in a variety of roles. Participants developed and evaluated a practice improvement plan based on survey results. Participants were also surveyed on their opinions on the feasibility of implementing the piloted survey instrument in their practices. Response rates for the patient care and the peer surveys were 15% and 75%, respectively. Both surveys indicated a high level of satisfaction with the diplomates; approximately two-thirds of physicians could not identify practice areas in need of improvement. These results suggest that threats to the validity of these surveys include distribution bias for peer surveys and response bias for patient surveys and that surveys often do not provide actionable information useful for practice improvement. Alternative approaches, such as including anesthesiologists within an integrated institutional evaluation system, could be explored to maximize the benefits of physician assessments provided by peers and patients. This purpose of this article is to update plastic and craniomaxillofacial surgeons on recent changes in treatment of cleft palate, provide a means for accurate self-assessment, and guide further individual in-depth study and development.

Practical Applications for Maintenance of Certification Products in Child and Adolescent Residency Training.

The authors evaluated whether Maintenance of Certification (MOC) Performance-in-Practice products in training increases trainee knowledge of MOC processes and is viewed by trainees as a useful activity. Six child and adolescent psychiatry fellowships used MOC products in continuity clinics to assess their usefulness as training tools. Two surveys assessed initial knowledge of MOC and usefulness of the activity. Forty-one fellows completed the initial survey. A majority of first-year fellows indicated lack of awareness of MOC in contrast to a majority of second-year fellows who indicated some awareness. Thirty-five fellows completed the second survey. A majority of first- and second-year fellows found the activity easy to execute and would change something about their practice as a result. Using MOC products in training appears to be a useful activity that may assist training programs in teaching the principles of self- and peer-learning.

The American Board of Internal Medicine Maintenance of Certification Examination and State Medical Board Disciplinary Actions: a Population Cohort Study.

Some have questioned whether successful performance in the American Board of Internal Medicine (ABIM) Maintenance of Certification (MOC) program is meaningful. The association of the ABIM Internal Medicine (IM) MOC examination with state medical board disciplinary actions is unknown. To assess risk of disciplinary actions among general internists who did and did not pass the MOC examination within 10 years of initial certification. Historical population cohort study. The population of internists certified in internal medicine, but not a subspecialty, from 1990 through 2003 (n = 47,971). ABIM IM MOC examination. General internal medicine in the USA. The primary outcome measure was time to disciplinary action assessed in association with whether the physician passed the ABIM IM MOC examination within 10 years of initial certification, adjusted for training, certification, demographic, and regulatory variables including state medical board Continuing Medical Education (CME) requirements. The risk for discipline among physicians who did not pass the IM MOC examination within the 10 year requirement window was more than double than that of those who did pass the examination (adjusted HR 2.09; 95% CI, 1.83 to 2.39). Disciplinary actions did not vary by state CME requirements (adjusted HR 1.02; 95% CI, 0.94 to 1.16), but declined with increasing MOC examination scores (Kendall's tau-b coefficient = - 0.98 for trend, p < 0.001). Among disciplined physicians, actions were less severe among those passing the IM MOC examination within the 10-year requirement window than among those who did not pass the examination. Passing a periodic assessment of medical knowledge is associated with decreased state medical board disciplinary actions, an important quality outcome of relevance to patients and the profession.

Maintenance of Certification: Perceptions and Attitudes of Neurosurgeons.

Assuring clinical competence throughout the career of a neurosurgeon is of paramount importance for patient safety. We present the first comprehensive survey of all neurosurgeons board certified through the American Board of Neurological Surgery (ABNS) to evaluate perceptions of Maintenance of Certification (MOC). We administered a validated, online, confidential survey to 4899 neurosurgeons (2435 ABNS diplomates participating in MOC, 1440 diplomates certified prior to 1999 [time-unlimited certificates], and 1024 retired diplomates). We received 1449 responses overall (30% response rate). Our study found that most respondents believe that neurosurgeons should be required to participate in continuing professional improvement following initial board certification (75%). Most believe that specialty boards, working in conjunction with specialty societies, should require diplomates to participate in programs meant to promote continuous professional development (73%). The majority of respondents (76%) believed that self-assessment tests constituted a meaningful professional development activity, in addition to periodic case log reviews (33%) or quality improvement projects (32.6%). A plurality of respondents (44%) do not feel that the MOC process as currently structured provides them with value. There were no differences between those who were "grandfathered" and those who actively participate in MOC and no differences between those in private practice versus those in academics. The ABNS is cognizant of diplomate concerns and is actively developing new MOC paradigms to ensure that the process achieves both the goals of meeting the public interest and assuring that the quality of American neurosurgery remains exemplary. Maintenance measures for preservation and recovery of permeable pavement surface infiltration rate--The effects of street sweeping, vacuum cleaning, high pressure washing, and milling.

The surface infiltration rates (SIR) of permeable pavements decline with time as sediment and debris clog pore spaces. Effective maintenance techniques are needed to ensure the hydraulic functionality and water quality benefits of this stormwater control. Eight different small-scale and full-scale maintenance techniques aimed at recovering pavement permeability were evaluated at ten different permeable pavement sites in the USA and Sweden. Maintenance techniques included manual removal of the upper 2 cm of fill material, mechanical street sweeping, regenerative-air street sweeping, vacuum street sweeping, hand-held vacuuming, high pressure washing, and milling of porous asphalt. The removal of the upper 2 cm of clogging material did not significantly improve the SIR of concrete grid paves (CGP) and permeable interlocking concrete pavers (PICP) due to the inclusion of fines in the joint and bedding stone during construction, suggesting routine maintenance cannot overcome improper construction. For porous asphalt maintenance, industrial hand-held vacuum cleaning, pressure washing, and milling were increasingly successful at recovering the SIR. Milling to a depth of 2.5 cm nearly restored the SIR for a 21-year old porous asphalt pavement to like-new conditions. For PICP, street sweepers employing suction were shown to be preferable to mechanical sweepers; additionally, maintenance efforts may become more intensive over time to maintain a threshold SIR, as maintenance was not 100% effective at removing clogging material. During the last century, the creation and implementation of board certification has had a powerful impact on the medical community. Board certification has helped to shape the scope and practice of medical professionals and the care they provide, as well as to influence the way the health insurance industry sets standards for reimbursement. One profession that offers board certification to its members is medical dosimetry. The Medical Dosimetrist Certification Board exam has been administered since 1988 and its content covers a broad spectrum of information from the radiation therapy sciences. The exam has strict application requirements and is rather difficultmore » to pass. Those who pass the exam can then call themselves Certified Medical Dosimetrists. For data purposes of this study, several members of the dosimetry community were solicited to participate in a survey regarding the exam's content and history, and to provide relevant statistical data. Currently 2,177 medical dosimetrists are board certified, with an additional 1,500 estimated to be working without certification. Although board certification is not currently required to practice medical dosimetry, new legislation known as the CARE Bill could change this. The CARE Bill, if passed, would mandate nationwide compulsory licensure and/or certification for medical dosimetrists and other medical professionals who want to work in radiation-related health care. Health maintenance organizations and other insurance carriers may likewise require certification for reimbursement purposes.« less

The Commercial Crew Program (CCP) is leading NASA's efforts to develop the next U.S. capability for crew transportation and rescue services to and from the International Space Station (ISS) by the mid-decade timeframe. The outcome of this capability is expected to stimulate and expand the U.S. space transportation industry. NASA is relying on its decades of human space flight experience to certify U.S. crewed vehicles to the ISS and is doing so in a two phase certification approach. NASA Certification will cover all aspects of a crew transportation system, including development, test, evaluation, and verification; program management and control; flight readiness certification; launch, landing, recovery, and mission operations; sustaining engineering and maintenance/upgrades. To ensure NASA crew safety, NASA Certification will validate technical and performance requirements, verify compliance with NASA requirements, validate the crew transportation system operates in appropriate environments, and quantify residual risks.

Systems1,2 required to exhibit high operational reliability often rely on some form of fault protection to recognize and respond to faults, preventing faults' escalation to catastrophic failures. Integrated System Health Management (ISHM) extends the functionality of fault protection to both scale to more complex systems (and systems of systems), and to maintain capability rather than just avert catastrophe. Forms of ISHM have been utilized to good effect in the maintenance phase of systems' total lifecycles (often referred to as 'condition-based mainte-nance'), but less so in a 'fault protection' role during actual operations. One of the impediments to such use lies in the challenges of verification, validation and certification of ISHM systems themselves. This paper makes the case that state-of-the-practice V&V and certification techniques will not suffice for emerging forms of ISHM systems; however, a number of maturing software engineering assurance technologies show particular promise for addressing these ISHM V&V challenges.

The Commercial Crew Program (CCP) is leading NASA's efforts to develop the next U.S. capability for crew transportation and rescue services to and from the International Space Station (ISS) by the middecade timeframe. The outcome of this capability is expected to stimulate and expand the U.S. space transportation industry. NASA is relying on its decades of human space flight experience to certify U.S. crewed vehicles to the ISS and is doing so in a two phase certification approach. NASA certification will cover all aspects of a crew transportation system, including: Development, test, evaluation, and verification. Program management and control. Flight readiness certification. Launch, landing, recovery, and mission operations. Sustaining engineering and maintenance/upgrades. To ensure NASA crew safety, NASA certification will validate technical and performance requirements, verify compliance with NASA requirements, validate that the crew transportation system operates in the appropriate environments, and quantify residual risks. The Commercial Crew Program will present progress to date and how it manages safety and reduces risk.

In 2002 the American Board of Radiology (ABR) discontinued issuing lifetime board certification. After that time diplomates received a timelimited certificate and must participate in the Maintenance of Certification (MOC) program in order to maintain their certification. Initially certificates were issued with a 10 year expiration period and the MOC had requirements to be met over that 10 year period. The goal was to demonstrate continuous maintenance of clinical competency, however some diplomates were attempting to fulfill most or all of the requirements near the end of the 10 year period. This failed to meet the continuous aspect of themore » goal and so the ABR changed to a sliding 3-year window. This was done to recognize that not every year would be the same, but that diplomates should be able to maintain a reasonable average over any 3 year period.A second significant change occurred in 2013. The initial requirements included 20 selfassessment modules (SAMs) over the original 10 year term. SAMs are a special type of continuing education (CE) credit that were an addition to the 250 standard CE credits required over the 10 year period. In 2013, however, the new requirement is 75 CE credits over the previous 3 years, of which 25 must include self-assessment. Effectively this raised the self-assessment requirement from 20 in 10 years to 25 in 3 years. Previously SAMs were an interactive presentation available in limited quantities at live meetings. However, the new requirement is not for SAMs but CE-SA which includes SAMs, but also includes the online quizzes provided at the AAPM online learning center. All credits earned at the AAPM online learning center fulfill the ABR SA requirement.This talk will be an interactive demonstration of the AAPM online learning center along with a discussion of the MOC requirements.« less

Performance of Certification and Recertification Examinees on Multiple Choice Test Items: Does Physician Age Have an Impact?

The development of recertification programs (now referred to as Maintenance of Certification or MOC) by the members of the American Board of Medical Specialties provides the opportunity to study knowledge base across the professional lifespan of physicians. Research results to date are mixed with some studies finding negative associations between age and various measures of competency and others finding no or minimal relationships. Four groups of multiple choice test items that were independently developed for certification and MOC examinations in psychiatry and neurology were administered to certification and MOC examinees within each specialty. Percent correct scores were calculated for each examinee. Differences between certification and MOC examinees were compared using unpaired t tests, and logistic regression was used to compare MOC and certification examinee performance on the common test items. Except for the neurology certification test items that addressed basic neurology concepts, the performance of the certification and MOC examinees was similar. The differences in performance on individual test items did not consistently favor one group or the other and could not be attributed to any distinguishable content or format characteristics of those items. The findings of this study are encouraging in that physicians who had recently completed residency training possessed clinical knowledge that was comparable to that of experienced physicians, and the experienced physicians' clinical knowledge was equivalent to that of recent residency graduates. The role testing can play in enhancing expertise is described.

CM and DM in an ISO R and D Environment

ISO 9000 - a common buzz word in industry is making inroads to government agencies. The National Aeronautics and Space Agency (NASA) achieved ISO 9001 certification at each of its nine (9) Centers and Headquarters in 1998-1999. NASA Glenn Research Center (GRC) was recommended for certification in September 1999. Since then, each of the Centers has been going through the semi-annual surveillance audits. Growing out of the manufacturing industry, successful application of the international quality standard to a research and development (R&D) environment has had its challenges. This paper will address how GRC applied Configuration Management (CM) and Data (or Document) Management (DM) to meet challenges to achieve ISO certification. One of the first challenges was to fit the ISO 9001-1994 elements to the GRC environment. Some of the elements fit well-Management Responsibility (4.1), Internal Audits (4.17), Document and Data Control (4.5). Other elements were not suited or applied easily to the R&D environment-Servicing (4.19), Statistical Techniques (4.20). Since GRC "builds" only one or two items at a time, these elements were considered not applicable to the environment.

Data Mining CMMSs: How to Convert Data into Knowledge.

Although the healthcare technology management (HTM) community has decades of accumulated medical device-related maintenance data, little knowledge has been gleaned from these data. Finding and extracting such knowledge requires the use of the well-established, but admittedly somewhat foreign to HTM, application of inferential statistics. This article sought to provide a basic background on inferential statistics and describe a case study of their application, limitations, and proper interpretation. The research question associated with this case study involved examining the effects of ventilator preventive maintenance (PM) labor hours, age, and manufacturer on needed unscheduled corrective maintenance (CM) labor hours. The study sample included more than 21,000 combined PM inspections and CM work orders on 2,045 ventilators from 26 manufacturers during a five-year period (2012-16). A multiple regression analysis revealed that device age, manufacturer, and accumulated PM inspection labor hours all influenced the amount of CM labor significantly (P < 0.001). In essence, CM labor hours increased with increasing PM labor. However, and despite the statistical significance of these predictors, the regression analysis also indicated that ventilator age, manufacturer, and PM labor hours only explained approximately 16% of all variability in CM labor, with the remainder (84%) caused by other factors that were not included in the study. As such, the regression model obtained here is not suitable for predicting ventilator CM labor hours.

Laminar-airflow equipment certification: what the pharmacist needs to know.

The basic information pharmacy practitioners need to determine the suitability and applicability of laminar-airflow equipment test standards and procedures is presented. The operative guideline for any laminar-flow clean bench (LFCB) certification is the cleanroom and work station requirements for controlled environments as defined by the federal government under Federal Standard 209b (FS 209b). FS 209b outlines the tests, test procedures, and acceptable performance ranges for all LFCB equipment. National Sanitation Foundation Standard Number 49 (NSF 49) is used in the certification of biological-safety cabinets (BSCs). NSF 49 covers those aspects of safety, maintenance, performance, and testing that are unique BSCs. To monitor certification properly, practitioners should be familiar with these standards and the air-velocity profile, high-efficiency particulate air filter performance, noise output, light, and electrical test procedures. A review of the requisite knowledge, experience, and reputation of certifying agents is presented, along with an outline of all the necessary procedures, equipment, and documentation to be used in the process. A thorough test report should be issued upon unit certification. As pharmacy practitioners are responsible for all other aspects of quality assurance, they should also be capable of auditing these certifications to ensure the aseptic quality of products compounded in the laminar-airflow environment.

The field of physical medicine and rehabilitation (PM&R) began its development in Taiwan since 1958, and has gradually evolved into a profession that provides a diversity of clinical services and educational activities. The objectives of this brief report were to (1) review its background and milestones in the past 6 decades, (2) describe the current role and function of its Academy of PM&R, (3) present the content and format of its board certification process, focusing on (a) residency training requirements, (b) self-assessment examination, board certification process, and (c) maintenance of certification through continued education. Some challenges and opportunities are also discussed toward the end of this article. It is hoped that this report may serve as a reference material for other countries as they develop or improve their PM&R residency programs.

This paper deals with the application of copula in maintenance contracts for a nonrepayable item. Failures of the item are modeled using a two dimensional approach where age and usage of the item and this requires a bi-variate distribution to modelling failures. When the item fails then corrective maintenance (CM) is minimally repaired. CM can be outsourced to an external agent or done in house. The decision problem for the owner is to find the maximum total profit whilst for the agent is to determine the optimal price of the contract. We obtain the mathematical models of the decision problems for the owner as well as the agent using a Nash game theory formulation.

The program is approved by both the American Board of Industrial Hygiene, and the Board of Certified Safety Professionals, which award Certification Maintenance (CM) points and Continuance of Certification (COC) points, respectively, for successful participation. Participants must read each issue of the Journal, answer "The Action Level"® questions, and return the completed answer sheet at the end of that issue's "The Action Level"® column. To earn the designated CM or COC credit, a score of 70 percent or better is required within a 12-month period. Certified Industrial Hygienists and Certified Associate Industrial Hygienists may earn 2 points per year. Certified Safety Professionals may earn 1.2 points per year. Enrollment is possible each month, but points are awarded only four times each year-in March, June, September, and December-to participants who score an average of 70 percent or better within each threemonth period. For example, if you register in December 2004, you will receive 0.5 CM points and/or 0.3 COC points for your January, February, and March 2005 responses (assuming that you submitted all answer sheets and scored a 70 percent or better average on them; only in your first quarter of enrollment will you be permitted to submit answer sheets for two months and receive full credit; three answer sheets are required for all other quarters). Nationwide, as physicians and health care systems adopt electronic health records, health information technology is becoming integral to the practice of medicine. But current medical education and professional development curricula do not systematically prepare physicians to use electronic health records and the data these systems collect. We detail how training in meaningful use of electronic health records could be incorporated into physician training, from medical school, through licensure and board certification, to continuing medical education and the maintenance of licensure and board certification. We identify six near-term opportunities for professional organizations to accelerate the integration of health information technology into their requirements.
