In 1994 the Department of Energy (Department) obtained the services of Rocky Mountain BankCard System, through the use of a General Services Administration contract, as a means for the Department and its contractors to make small purchases. The use of credit cards was expected to simplify small purchase procedures and improve cash management. The Ohio Field Office (Field Office) uses the credit card system and oversees usage by its area offices. Contractors under the Field Office also use the credit card system to make small purchases. The Office of Inspector General (OIG) has issued one audit report concerning the usemore » of credit cards. In April 1996, the OIG issued Report WR-B-96-06, Audit of Bonneville Power Administration`s Management of Information Resources. The audit concluded that improvements could be made in implementing credit card and property procedures in Bonneville`s management of computer-related equipment. Specifically, many credit card purchases were made by employees whose authority to buy was not properly documented, and the purchasing files often lacked invoices that would show what was purchased. Additionally, some cardholders split purchases to avoid credit card limits. The objective of this audit was to determine whether the Field Office, Fernald and Miamisburg Environmental Management Projects, Fluor Daniel, and B and W were using credit cards for the appropriate purposes and within the limitations established by Federal and Departmental regulations.« less

In 2000 the Skåne Region (a public authority) and a private contractor made a five-year agreement for the provision of both in-patient care and out-patient medical services to about 30,000 inhabitants in the south-east part of the region. The Skåne Region is the main provider of health care to about one million inhabitants in the south of Sweden and is responsible for all health care (private and public), including ten hospitals. This paper seeks to answer the question of how the Skåne Region can control and cooperate with a private contractor, entering into competition with the public health care providers in the region. This is a longitudinal study conducted between 2001-2006. It is based on 28 taped interviews with employees responsible for the contracting process, participating observations and comprehensive secondary material. The study presents experiences made by the contractor and the public authority on how to work out and follow-up assignments within the health care sector regarding patient interest, public interest and professional medical interest. Measurement within the frames of the balanced scorecard (BSC) made it possible to control both volumes and health care quality delivered by the private competing contractor. The political purchaser claims that the Skåne Region has established a cost-effective and successful control system based on trust and measurement. This paper reports on a control system, between public purchaser and a private provider within health care, that focuses on and follow-up not only health care production but also health care quality.

Aquila oil field is located in 850 meters of water in the middle of the Otranto Channel, in the Mediterranean Sea, at about 45 km from the shore and is subject to both difficult sea and weather conditions. The many difficulties, mainly due to the very high water depth, imposed the use of advanced technology, that could be obtained only through the direct association of contractor companies, leaders in their own field. Such a solution safeguards the technological reliability and allows the maximum control of time and cost. The selection of an FPSO (Floating, Production, Storage and Offloading) comes frommore » a feasibility study indicating this solution as the only one, allowing the economical exploitation of the Aquila field. This paper deals with a series of technical solutions and contractual agreements with a Joint-Venture embracing two leading world contractors for developing, manufacturing and installing the FPSO {open_quotes}Agip Firenze{close_quotes}, permanently anchored at a world record 850 m water depth. The transformation to managed care is one of the most important and complex changes ever to take place in the American health system. One key aspect of this transformation is its implications for public health policy and practice. Both public and private buyers purchase managed care; increasingly, public programs that used to act as their own insurers (i.e., Medicare, Medicaid and CHAMPUS) are purchasing large quantities of managed care insurance from private companies. The transformation to managed care is altering the manner in which public health policy makers conceive of and carry out public health activities (particularly activities that involve the provision of personal health services). The degree to which managed care changes public health and in turn is altered by public health will depend in great measure on the extent to which public and private policy makers understand the implications of their choices for various aspects of public health and take steps to address them. Because both publicly and privately managed care arrangements are relatively deregulated, much of the dialogue between public health and managed care purchasers can be expected to take place within the context of the large service agreements that are negotiated between buyers and sellers of managed care products. This is particularly true for Medicaid because of the importance of Medicaid coverage, payment and access policies to public health policy makers, and because of the public nature of the Medicaid contracting process. A nationwide study of Medicaid managed care contracts offers the first detailed analysis of the content and structure of managed care service agreements and the public health issues they raise. Four major findings emerge from a review of the contracts. First, most of the agreements fail to address key issues regarding which Medicaid-covered services and benefits are the contractor's responsibility and which remain the residual responsibility of the state agency. Second, most contracts fail to address the legal and structural issues arising from the relationship between the managed care service system and the public health system, including such key matters as access to care for communicable diseases and contractors' relationship to state public health laboratories. Third, many contracts are silent on health agencies' access to data for surveillance and community health measurement purposes. Finally, many contracts may be developed with only a limited understanding of the key public health-related issues facing the community from which the members will be drawn. The CDC and state and local public health agencies must expand their activities in the area of managed care contract specifications. For several years the CDC has been involved in an ongoing effort to develop quality of care measures to be collected from all companies through the HEDIS process. As important as this effort is, it represents only an attempt to measure what managed care does rather than an a priori effort to shape the underlying policy and organizational structure of managed care itself. Integrating managed care with public health policy will require this type of affirmative effort with both Medicaid agencies as well as other managed care purchasers.

The Technical Work Plan Tracking Tool is a web-based application that enables interactive communication and approval of contract requirements that pertain to the administration of the Science, Engineering, Analysis, and Test (SEAT) contract at Johnson Space Center. The implementation of the application has (1) shortened the Technical Work Plan approval process, (2) facilitated writing and documenting requirements in a performance-based environment with associated surveillance plans, (3) simplified the contractor s estimate of the cost for the required work, and (4) allowed for the contractor to document how they plan to accomplish the work. The application is accessible to over 300 designated NASA and contractor employees via two Web sites. For each employee, the application regulates access according to the employee s authority to enter, view, and/or print out diverse information, including reports, work plans, purchase orders, and financial data. Advanced features of this application include on-line approval capability, automatic e-mail notifications requesting review by subsequent approvers, and security inside and outside the firewall.

The Hunger Prevention and Nutrition Assistance Program (HPNAP) is a New York State Department of Health program. The HPNAP improves nutritional quality of food available at food banks, food pantries, soup kitchens, and emergency shelters through contractual relationships to fund the purchase, delivery, storage, and service of nutritious food. To determine whether a one-time fiscal stimulus of the Locally Grown Produce Initiative to HPNAP contractors in 2012-2013 would result in a short-term sustainable increase in the proportion of dollars spent on New York State Grown (NYSG) produce. Quasi-experimental, nonequivalent control group design. We analyzed New York State Department of Health administrative data regarding expenditures on all produce and NYSG produce by HPNAP contractors. New York State. The proportion of dollars spent on NYSG produce during 2011-2012 (preintervention) and 2013-2014 (postintervention) was compared between HPNAP food bank contractors (recipients of stimulus money, n = 8) and non-food bank contractors (nonrecipients, n = 34) using nonparametric methods. The HPNAP Locally Grown Produce Initiative was associated with an increased proportion of NYSG produce spending by food bank contractors that received a fiscal stimulus 1 year later. Upstate food banks had the largest increase (median 31.6%) among all HPNAP contractors. The results of this study revealed that the Locally Grown Produce Initiative fiscal stimulus had a positive, year-long and statewide effect on the proportion of expenditure on NYSG produce by food banks. We hope that the initial success seen in New York State may encourage other states to adopt similar initiatives in future.

Department of Energy, Richland Operations Office (DOE-RL) has initiated Phase 1 of a two-phase privatization strategy for treatment and immobilization of low-activity waste (LAW) currently being managed by the Hanford Tank Waste Remediation System (TWRS) Project. In this strategy, DOE will purchase services from a contractor-owned and operated facility under a fixed price. The Phase 1 TWRS privatization contract requires that the Project Hanford Management Contract (PHMC) contractors, on behalf of DOE, deliver LAW feed in specified quantities and composition to the Privatization Contractor in a timely manner (DOE-RL 1996). Additional requirements are imposed by the interface controlmore » document (ICD-19) for LAW feed (PHMC 1997). In response to these requirements, the Tank Waste Remediation System Operation and Utilization Plan (TWRSO and UP) (Kirkbride et al. 1997) was prepared by the PHMC. The TWRSO and UP, as updated by the Readiness-To-Proceed deliverable (Payne et al. 1998), establishes the baseline operating scenario for the delivery of LAW feed to the Privatization Contractor. The scenario specifies tanks from which LAW will be provided for each feed batch, the operational activities needed to prepare and deliver each batch, and the timing of these activities. The operating scenario was developed based on current knowledge of waste composition and chemistry, waste transfer methods, and operating constraints, such as tank farm logistics and availability of tank space. A project master baseline schedule (PMBS) has been developed to implement the operating scenario. The PMBS also includes activities aimed at reducing programmatic risks. One of the activities, ''Confirm Plans and Requirements,'' was identified to verify the basis used to develop the scenario. Additional data on waste quantity, physical and chemical characteristics, and transfer properties will be needed to support this activity. This document describes the data quality objective (DQO) process undertaken to assme appropriate data will be collected to support the activity, ''Confirm Tank Plans and Requirements.'' The U.S. Environmental Protection Agency, either directly or through its Superfund contractors, is a major user of chemical protective clothing. The purpose of the study was to develop estimates for the cost of using this clothing. These estimates can be used to guide purchase decisions and use practices. For example, economic guidelines would assist in decisions pertinent to single-use versus reusable clothing. Eight cost elements were considered: (1) purchase cost, (2) the number of times an item is used, (3) the number of items used per day, (4) cost of decontamination, (5) cost of inspection, (6) cost of maintenance, (7)more » cost of storage, and (8) cost of disposal. Estimates or assumed inputs for each of these elements were developed based on labor costs, fixed costs, and recurring costs. The cost elements were combined into an economic (mathematical) model having the single output of cost/use. By comparing cost/use for various use scenarios, conclusions are readily reached as to the optimum economics for purchase, use, and reuse of the clothing. In general, clothing should be considered disposable if its purchase cost is less than its average cost/use per use for the anticipated number of times it will be reused.« less

The U.S. Department of Energy-Richland Operations Office (DOE-RL) has initiated Phase 1 of a two-phase privatization strategy for treatment and immobilization of high-level waste (HLW) that is currently managed by the Hanford Tank Waste Remediation System (TWRS) Project. In this strategy, DOE will purchase services from a contractor-owned and operated facility under a fixed price. The Phase 1 TWRS privatization contract requires that the Project Hanford Management Contract (PHMC) contractors, on behalf of DOE, deliver HLW feed in specified quantities and composition to the Privatization Contractor in a timely manner (DOE-RL 1996). Additional requirements are imposed by the interface controlmore » document (ICD) for HLW feed (PHMC 1997). In response to these requirements, the Tank Waste Remediation System Operation and Utilization Plan (TWRSO and UP) (Kirkbride et al. 1997) was prepared by the PHMC. The TWRSO and UP, as updated by the Readiness-To-Proceed (RTP) deliverable (Payne et al. 1998), establishes the baseline operating scenario for the delivery of HLW feed to the Privatization Contractor. The scenario specifies tanks from which HLW will be provided for each feed batch, the operational activities needed to prepare and deliver each batch, and the timing of these activities. The operating scenario was developed based on current knowledge of waste composition and chemistry, waste transfer methods, and operating constraints such as tank farm logistics and availability of tank space. A project master baseline schedule (PMBS) has been developed to implement the operating scenario. The PMBS also includes activities aimed at reducing programmatic risks. One of the activities, ''Confirm Tank TI is Acceptable for Feed,'' was identified to verify the basis used to develop the scenario Additional data on waste quantity, physical and chemical characteristics, and transfer properties will be needed to support this activity. This document describes the data quality objective (DQO) process undertaken to assure appropriate data will be collected to support the activity, ''Confirm Tank T is Acceptable for HLW Feed.'' The DQO process was implemented in accordance with the TWRS DQO process (Banning 1997) with some modifications to accommodate project or tank-specific requirements and constraints.« less

Formal and legal aspects of buying flats and their reception is very current topic and touches wide group of buyers. Annually in Poland great amount of flats is being sold and put to use. However, the case of housing purchase requires knowledge of both the construction and the legal aspects each buyer has to encounter. The paper faces the subject of formal and legal aspects, and analyses accompanying procedure of purchase and reception of housing in Poland. The article presents principles associated with the acquisition of a dwelling, process of works reception, removal of detected faults, fault-free reception, transfer of ownership, warranties, guarantees and possibilities of their enforcement. Contracting parties of the developer agreement were revealed. In addition, the entities present in the course of works such as general contractor were mentioned, due to the fact of his direct influence on the results of a contract terms between developer and buyer. Logical connection between three parties (buyer-developer-general contractor) were shown and direct and indirect dependencies were revealed. Existing laws and regulations that govern the relationship between the developer and the buyer of a dwelling were determined showing basic rights and responsibilities of each. The article also presents problems resulting from delaying the completion of works by developer’s fault and indicates possible legal paths to follow in order claim their rights. Due to the fact, that many of discussed formal and legal aspects in this subject have their origin connected to construction works and design issues, author suggests increased quality control and efficient work organization in order to solve problems before appearance.

The use of performance-based contracting at Kennedy Space Center has necessitated a shift from intrusive oversight of contractor activities to an insight surveillance role. This paper describes the results of a pilot implementation of the NASA Quality Surveillance System (NQSS) in the Space Shuttle Main Engines Processing Facility. The NQSS is a system to sample contractor activities using documented procedures, specifications, drawings and observations of work in progress to answer the question "Is the contractor doing what they said they would do?" The concepts of the NQSS are shown to be effective in providing assurance of contractor quality. Many of the concepts proven in the pilot are being considered for incorporation into an overall KSC Quality Surveillance System.

This paper describes the results of a pilot implementation of the NASA Quality Surveillance System (NQSS) in the Space Shuttle Main Engines Processing Facility. The NQSS is a system to sample contractor activities using documented procedures, specifications, drawings and observations of work in progress to answer the question "Is the contractor doing what they said they would do?" The concepts of the NQSS are shown to be effective in providing assurance of contractor quality. Many of the concepts proven in the pilot are being considered for incorporation into an overall KSC Quality Surveillance System.

Ten advanced energy conversion systems for central-station, based-load electric power generation using coal and coal-derived fuels which were studied by NASA are presented. Various contractors were selected by competitive bidding to study these systems. A comparative evaluation is provided of the contractor results on both a system-by-system and an overall basis. Ground rules specified by NASA, such as coal specifications, fuel costs, labor costs, method of cost comparison, escalation and interest during construction, fixed charges, emission standards, and environmental conditions, are presented. Each system discussion includes the potential advantages of the system, the scope of each contractor's analysis, typical schematics of systems, comparison of cost of electricity and efficiency for each contractor, identification and reconciliation of differences, identification of future improvements, and discussion of outside comments. Considerations common to all systems, such as materials and furnaces, are also discussed. Results of selected in-house analyses are presented, in addition to contractor data. The results for all systems are then compared.

Employers are demanding information about the performance of the health care plans they purchase for their employees. As a result, "report cards" are now beginning to appear that provide standardized, population-based comparison data for managed medical care plans' quality of care, access and member satisfaction, utilization, and financial status. Although report cards for dental care plans have not yet been developed, it is likely that purchasers will soon expect such performance information. A prototype report card for dental managed care plans is proposed in an effort to facilitate the development of a consensus standard for dentistry. The thirty-eight measures proposed for the report card are designed to be obtainable with a realistic level of additional effort in most dental practices. They were selected to provide data on questions of importance to purchasers and to assess processes and outcomes important because there is strong evidence for their effectiveness. The rationale for the measures is discussed, as are the steps required to develop more sophisticated measures. While the responsibility for the procurement of the information needed for dental report cards will die initially with administrators of dental care plans, it is likely in the near future that individual practitioners will be expected to supply this information to both individual patients and potential contractors.

Strategic Purchasing in Practice: Comparing Ten European Countries.

Strategic purchasing of health care services is widely recommended as a policy instrument. We conducted a review of literature of material drawn from the European Observatory on Health Systems and Policies Health Systems in Transition series, other European Observatory databases, and selected country-specific literature to augment the comparative analysis by providing the most recent healthcare trends in ten selected countries. There is little evidence of purchasing being strategic according to any of the established definitions. There is little or no literature suggesting that existing purchasing mechanisms in Europe deliver improved population health, citizen empowerment, stronger governance and stewardship, or develop purchaser organization and capacity. Strategic purchasing has not generally been implemented. Policymakers considering adopting strategic purchasing policies should be aware of this systemic implementation problem. Policymakers in systems with strategic purchasing built into policy should not assume that a purchasing system is strategic or that it is delivering any expected objectives. However, there are individual components of strategic purchasing that are worth pursuing and can provide benefits to health systems. Copyright © 2018. Published by Elsevier B.V.

The Lunar Reconnaissance Orbiter (LRO) is the first mission of the Robotic Lunar Exploration Program (RLEP), a space exploration venture to the Moon, Mars and beyond. The LRO mission includes spacecraft developed by NASA Goddard Space Flight Center (GSFC) and seven instruments built by GSFC, Russia, and contractors across the nation. LRO is defined as a measurement mission, not a science mission. It emphasizes the overall objectives of obtaining data to facilitate returning mankind safely to the Moon in preparation for an eventual manned mission to Mars. As the first mission in response to the President's commitment of the journey of exploring the solar system and beyond: returning to the Moon in the next decade, then venturing further into the solar system, ultimately sending humans to Mars and beyond, LRO has high-visibility to the public but limited resources and a tight schedule. This paper demonstrates how NASA's Lunar Reconnaissance Orbiter Mission project office incorporated reliability analyses in assessing risks and performing design tradeoffs to ensure mission success. Risk assessment is performed using NASA Procedural Requirements (NPR) 8705.5 - Probabilistic Risk Assessment (PRA) Procedures for NASA Programs and Projects to formulate probabilistic risk assessment (PRA). As required, a limited scope PRA is being performed for the LRO project. The PRA is used to optimize the mission design within mandated budget, manpower, and schedule constraints. The technique that LRO project office uses to perform PRA relies on the application of a component failure database to quantify the potential mission success risks. To ensure mission success in an efficient manner, low cost and tight schedule, the traditional reliability analyses, such as reliability predictions, Failure Modes and Effects Analysis (FMEA), and Fault Tree Analysis (FTA), are used to perform PRA for the large system of LRO with more than 14,000 piece parts and over 120 purchased or contractor built components.

Strategic purchasing in healthcare services is a key component in improving health system performance, and it has been one of the most important issues in health system reform around the world, especially Europe in the last decade. Iran health system and insurance, although sometimes considered the issue of strategic purchasing goals, has not been made possible to achieve or even to implement, due to the associated problems. To determine the associated problems of strategic purchasing in the Iran Health Insurance Organization (IHIO). This study is a qualitative study, and framework analysis which was conducted in Iran in 2014-15. The participants in this study were 34 individuals from decision-makers and executives in the IHIO purchasing process, and university experts who have been chosen purposefully. This study conducted frame analysis, by using MAXQDA 10. The findings included associated problems of IHIO strategic purchasing in 12 themes and 65 subthemes. The themes included: Laws and regulations for purchasing, Organization of purchasing, Qualified and authorized providers, Right type of services, Right type of contracts, Target groups for purchasing, Resources allocation, financing and pricing system, Purchasing as improving performance and quality, Purchasing as shaping the market and competition, Purchasing as health progress state of people and society, Guided purchasing and stewardship of government, Structure of decision-making process in the health and welfare ministries. The findings of this study showed associated problems in IHIO strategic purchasing. To achieve strategic purchasing goals in Iran, identification of all issues and factors of the total insurers and health system sets which affect strategic purchasing is essential.

The challenges of strategic purchasing of healthcare services in Iran Health Insurance Organization: a qualitative study

Background Strategic purchasing in healthcare services is a key component in improving health system performance, and it has been one of the most important issues in health system reform around the world, especially Europe in the last decade. Iran health system and insurance, although sometimes considered the issue of strategic purchasing goals, has not been made possible to achieve or even to implement, due to the associated problems. Objective To determine the associated problems of strategic purchasing in the Iran Health Insurance Organization (IHIO). Methods This study is a qualitative study, and framework analysis which was conducted in Iran in 2014–15. The participants in this study were 34 individuals from decision-makers and executives in the IHIO purchasing process, and university experts who have been chosen purposefully. This study conducted frame analysis, by using MAXQDA 10. Results The findings included associated problems of IHIO strategic purchasing in 12 themes and 65 subthemes. The themes included: Laws and regulations for purchasing, Organization of purchasing, Qualified and authorized providers, Right type of services, Right type of contracts, Target groups for purchasing, Resources allocation, financing and pricing system, Purchasing as improving performance and quality, Purchasing as shaping the market and competition, Purchasing as health progress state of people and society, Guided purchasing and stewardship of government, Structure of decision-making process in the health and welfare ministries. Conclusion The findings of this study showed associated problems in IHIO strategic purchasing. To achieve strategic purchasing goals in Iran, identification of all issues and factors of the total insurers and health system sets which affect strategic purchasing is essential. PMID:29629051
