City curfew ordinances and teenage motor vehicle injury.

Several U.S. cities have curfew ordinances that limit the late night activities of minor teenagers in public places including highways. Detroit, Cleveland, and Columbus, which have curfew ordinances, were compared to Cincinnati, which does not have such an ordinance. The curfew ordinances were associated with a 23% reduction in motor vehicle related injury for 13- to 17-year-olds as passengers, drivers, pedestrians, or bicyclists during the curfew hours. It was concluded that city curfew ordinances, like the statewide driving curfews studied in other states, can reduce motor vehicle injury to teenagers during the particularly hazardous late night hours.
Night driving curfews in New York and Louisiana: results of a questionnaire survey.

New York and Louisiana, two states having night driving curfews, were included in a questionnaire survey of high school students conducted in 1983. Most students in these states knew about their curfew law. In both states the majority of licensed drivers said they had violated the curfew, but the majority claimed to have violated the curfew no more than twice. Students do not believe that the police enforce curfew restrictions vigorously, but many parents require their children to obey the law. New York students were more knowledgeable than Louisiana students about the restrictions, and there was more evidence in New York than Louisiana of reduced driving resulting from the curfew. These findings are consistent with results of an earlier study that found curfews in both of these states reduced crashes but estimated crash reductions during curfew hours were greater in New York than in Louisiana.
Juvenile curfews: are they an effective and constitutional means of combating juvenile violence?

Curfew ordinances have become a popular way to attempt to combat juvenile crime and victimization. Although the Supreme Court has yet to hear a curfew case, several constitutional challenges have been brought in lower federal courts. The cases are replete with psychological assumptions for which there is limited empirical evidence. In applying the "strict scrutiny" standard, several courts have also questioned whether juvenile curfews are narrowly tailored to further the State's interest in reducing juvenile crime and victimization. While public opinion and reports from several police jurisdictions support the utility of juvenile curfews, recent empirical studies indicate that curfews are not effective at reducing juvenile offending or victimization. This paper argues that the emerging evidence does not support the use of juvenile curfews and urges policy makers and the courts to examine the efficacy of curfew legislation. Directions for future research that could be helpful to the courts in applying the Bellotti factors to curfew cases are also suggested. Copyright 2001 John Wiley & Sons, Ltd.
The Washington, DC, Youth Curfew: effect on transports of injured youth and homicides.

Curfews are implemented to curtail youth violence. Trauma systems and emergency medical services (EMS) may need to prepare for changes in patient volume resulting from local ordinances. This study evaluated the impact of the 1995 Washington, DC, Juvenile Curfew Act on EMS transports of injured youth and on youth homicides. A retrospective, comparative cohort study was performed. Transports of injured youth and youth homicides were counted in corresponding months of 1994 and 1995. Cohorts were formed by year, time of day, age group, and mechanism of injury. Year-to-year statistical comparisons of injury proportions were performed using the chi square and Fisher's exact tests. One thousand forty-eight transports were included. No significant difference was observed in transports with curfew implementation. Most assaults on youth occurred outside the curfew time. No effect of the curfew on homicides was detected. Of 67 homicides, only two victims were under the curfew. No effect of the curfew on transports for injuries or on homicides was demonstrated. The curfew was not in effect during the period of highest risk.

This research asked the following: (a) What is the effect of enacting a juvenile curfew law on youth drinking, and (b) do demographic variables moderate the relation between juvenile curfew law enactment and drinking? This study examined the effect of juvenile curfew laws on underage drinking, using data from 46 U.S. cities from 1991 to 2005. In 2014, we compiled a data set containing alcohol and curfew law data by zip code. It included 63,081 minors (ages 12-17 years) from 1,081 zip codes. We used difference-in-difference regressions to analyze the data. The effect of the enactment of a curfew law on the likelihood of consuming alcohol in the past year or past 30 days or of heavy episodic drinking in the past 2 weeks was not significant when compared with cities without curfew laws during the same periods. Although the likelihood of consuming alcohol over the past year differed depending on an individual's characteristics (e.g., race/ethnicity, age, and gender), none of the interaction terms between these characteristics and curfew laws were significant. Curfew laws appear to have a nonsignificant effect on youth drinking, but these results are unclear without more knowledge as to where and when youth are drinking both before and after the enactment of curfew laws and how these laws are being enforced.
The effect of city curfew ordinances on teenage motor vehicle fatalities.

Of 149 large cities surveyed, 72 were found to have nighttime curfew ordinances applying to teenagers of various ages. The typical city curfew identified starts at midnight, ends at 5 A.M., and affects all activity of teenagers ages 13-17 in a public place and unaccompanied by a parent. In comparisons of 47 cities with curfews covering 13- to 17-year-olds and 77 cities without curfews, curfews were associated with a 23% reduction in fatal injury for 13- to 17-year-olds for the 9 P.M.-5:59 A.M. time period. This is identical to the 23% reduction estimated for both fatal and nonfatal motor vehicle injuries for 13- to 17-year-olds in an earlier, more limited study of curfews in Detroit, Cleveland, and Columbus, Ohio.

Effect of a curfew law on juvenile trauma.

As a method of crime reduction among teenagers, several cities, counties, and states across the country have enacted, or attempted to enact, curfew laws. A curfew law was successfully implemented in Dade County, Florida, in January of 1996. Although its efficacy for crime reduction has been questioned, its benefit for trauma prevention may be real. Trauma registry data was collected retrospectively and prospectively from Dade County's Level I trauma center for all trauma victims 5 to 16 years of age. The time period spanned the 2 years before the institution of the curfew law (January 1, 1996) and the 2 years after. Total adult and pediatric trauma volumes during the 4-year period were used as comparisons, as well as juvenile traumas occurring during noncurfew hours. Total trauma volume did not change significantly across the 4-year period, nor did the volume among the curfew age group during noncurfew hours. The predominant mechanisms of injury during curfew hours were motor vehicle crashes and gunshots. Neither the patterns of mechanisms nor ages changed significantly during the precurfew and postcurfew eras. However, the volume of cases seen at the trauma center among the curfew age group was significantly lower with the curfew law in effect (mean, 7.0/month) than before it was in effect (mean, 9.5/month, p = 0.043). Although the overall trauma admissions and juvenile trauma admissions during the noncurfew hours remained relatively stable, juvenile trauma admissions during curfew hours dropped significantly in the 2 years after enforcement of the curfew law compared with the 2 years before the curfew law. This finding suggests that attempts to prevent late-night nonproductive street presence among teens can decrease the incidence of trauma occurrences.

Context Automobile crashes cause more than 800,000 youth injuries and deaths each year. Other youth suffer the consequences from being either a perpetrator or victim of a crime. One type of law that has an effect on youth behavior is juvenile curfew laws. These laws restrict the times that youth may occupy public places or streets. We systematically reviewed studies evaluating the effectiveness of these laws to address the question: Can juvenile curfew laws be used to improve youth public health and juvenile justice outcomes? Evidence acquisition In 2013, we used a standardized set of keywords to search 24 databases for studies that evaluated effectiveness outcome measures of juvenile curfew laws. After applying the exclusion criteria and removing duplicate studies, 14 studies of juvenile curfews remained. Evidence synthesis Of the six studies examining the effectiveness of juvenile curfew laws on adverse youth health outcomes, five found a positive impact. Of the eight studies examining the effectiveness of curfew laws on juvenile crime and victimization, four found a positive impact. Conclusions The studies that found that juvenile curfew laws were effective at reducing adverse youth health outcomes (e.g., trauma transports), juvenile crime, and victimization were of higher quality (e.g., stronger methodologic approaches) than those finding no effects. However, given the limited number of studies and concerns with quality, we conclude that more research is needed before conclusions can be drawn about the effectiveness of juvenile curfew laws. Night driving restrictions for youthful drivers: a literature review and commentary.

The research literature on night driving curfews is reviewed. Driving at night involves high risk, particularly for young beginners. Although only about 15 percent of the total miles of 16-17-year-old drivers occur between 9 p.m. and 6 a.m., about 40 percent of their fatal crashes take place during these hours. Curfews that limit recreational driving at night without an adult have been found to substantially reduce nighttime crashes. Parents of teenagers strongly endorse curfews and favor earlier starting times than prevail in most jurisdictions with curfews. A night driving curfew is an essential component of graduated licensing, a system that phases in young beginners to full-privilege licensure, limiting initial driving to lower-risk situations.

This study examines the effect of state driving age, learning permit, driver's education, and curfew laws on 15-17-year-old driver fatality rates. A multivariate regression model is estimated for 47 states and nine years. The minimum legal driving age and curfew laws are found to be important determinants of fatalities. Driver's education and learning permits have smaller effects. The relationship between rates of licensure and driving age, education, and curfew laws is also examined. In each case, a more restrictive policy is found to reduce licensure of 15-17 year olds. The results suggest that the imposition of curfew laws and higher minimum driving ages are particularly effective traffic safety policies.

The effect of a curfew on pediatric out-of-hospital EMS responses.

The purpose of a curfew is to decrease the amount of crime inflicted on minors during the late hours of the night. On June 1, 1994, a city curfew was instituted in New Orleans, requiring all persons 17 years of age or younger to be off the streets from 9 PM to 6 AM Sunday through Thursday, and from 11 PM to 6 AM on Friday and Saturday. This study evaluated the effect of the curfew on emergency medical services (EMS) transports for patients who were 17 years old or younger (pediatric). Data from all pediatric transports were included from the months before (5/94) and after (6/94) the institution of the curfew, and from the same two months one year earlier (5/93 and 6/93). A chi-square test was used to evaluate comparisons. The city EMS transports 48,000 patients per year in a one-tiered system (paramedic only) that acts as the sole provider of emergency EMS transport in the city. Approximately 10% of all transports are pediatric, and 40% of the pediatric transports are for trauma. A total of 1,642 transports were found that fit the inclusion criteria. In May 1993, there were 415 total pediatric transports; 234 were pediatric trauma. In June 1993, there were 406 total pediatric transports; 250 were pediatric trauma. In May 1994, there were 447 total pediatric runs; 243 were pediatric trauma. During the postcurfew month, June 1994, there were a significant decrease in pediatric transports to 370 (p < 0.01) and a significant decrease in pediatric trauma transport to 189 (p < 0.01). The institution of a curfew may lead to a drop in pediatric EMS runs during curfew hours. Another value of the curfew may be in the secondary effects of the curfew in preventing childhood injury during noncurfew hours.

The current study examined 14 waves of data derived from a large, community-based study of the sexual behavior of impoverished youth between 12 and 17 years of age residing in the Deep South. We used multilevel linear modeling to identify ecological predictors of intercourse frequency and number of sexual partners among gender-specific subsamples. Results indicated that predictors of adolescent sexual behavior differed by both type of sexual behavior and gender. For males, age, maternal warmth, parental knowledge, curfew, self-worth, and sense of community predicted intercourse frequency, while age, parental knowledge, curfew, self-worth, friend support, and sense of community were significantly associated with having multiple sexual partners. Among females, age, curfew, and self-worth exerted significant effects on intercourse frequency, while age, parental knowledge, curfew, and self-worth exerted significant effects on having multiple sexual partners. Aircraft noise disturbs sleep and impairs recuperation. Authorities plan to expand Frankfurt airport. To quantitatively assess the effects of a traffic curfew (11 PM to 5 AM) at Frankfurt Airport on sleep structure. Experimental sleep study; polysomnography for 13 consecutive nights. Sleep laboratory. Subjects. 128 healthy subjects, mean age (SD) 38 (13) years, range 19 to 65, 59% female. Intervention. Exposure to aircraft noise via loudspeakers. A 6-state Markov state transition sleep model was used to simulate 3 noise scenarios with first-order Monte Carlo simulations: 1) 2005 traffic at Frankfurt Airport, 2) as simulation 1 but flights between 11 PM and 5 AM cancelled, and 3) as simulation 2, with flights between 11 PM and 5 AM from simulation 1 rescheduled to periods before 11 PM and after 5 AM. Probabilities for transitions between sleep stages were estimated with autoregressive multinomial logistic regression. Compared to a night without curfew, models indicate small improvements in sleep structure in nights with curfew, even if all traffic is rescheduled to periods before and after the curfew period. For those who go to bed before 10:30 PM or after 1 AM, this benefit is likely to be offset by the expected increase of air traffic during late evening and early morning hours. Limitations. Limited ecologic validity due to laboratory setting and subject sample. According to the decision analysis, it is unlikely that the proposed curfew at Frankfurt Airport substantially benefits sleep structure. Extensions of the model could be used to evaluate or propose alternative air traffic regulation strategies for Frankfurt Airport.

Previous studies have shown that delaying licensure reduces motor vehicle injuries. This study examined the relationship between licensing laws and the timing of obtaining drivers licenses. The states studied have various laws, including minimum licensing ages of 15, 16 and 17; minimum learners permit ages of 15 and 16; and teenage night driving curfews. A questionnaire survey of 52,304 students was conducted in 75 high schools during the spring of 1983. The schools were selected from Orange County, California; rural Colorado; and most or all of Louisiana, Michigan, Mississippi, New Jersey and New York. The results indicated that the minimum age of licensure has obvious strong effects in delaying licensure. Setting the learners permit age later also apparently delays licensure. The night driving curfew in New York State (9 p.m.-5 a.m.) is seen by students as a factor in their decisions to delay obtaining licenses; Louisiana's 11 p.m.-5 a.m. curfew has a much smaller effect, if any.

Much controversy surrounds government regulation of routing and scheduling of Hazardous Materials Transportation (HMT). Increases in operating costs must be balanced against expected benefits from local HMT bans and curfews when promulgating or preempting HMT regulations. Algorithmic approaches for evaluating HMT routing and scheduling regulatory policy are described. A review of current US HMT regulatory policy is presented to provide a context for the analysis. Next, a multiobjective shortest path algorithm to find the set of efficient routes under conflicting objectives is presented. This algorithm generates all efficient routes under any partial ordering in a single pass through the network.more » Also, scheduling algorithms are presented to estimate the travel time delay due to HMT curfews along a route. Algorithms are presented assuming either deterministic or stochastic travel times between curfew cities and also possible rerouting to avoid such cities. These algorithms are applied to the case study of US highway transport of spent nuclear fuel from reactors to permanent repositories. Two data sets were used. One data set included the US Interstate Highway System (IHS) network with reactor locations, possible repository sites, and 150 heavily populated areas (HPAs). The other data set contained estimates of the population residing with 0.5 miles of the IHS and the Eastern US. Curfew delay is dramatically reduced by optimally scheduling departure times unless inter-HPA travel times are highly uncertain. Rerouting shipments to avoid HPAs is a less efficient approach to reducing delay.« less
Effectiveness of graduated driver licensing in reducing motor vehicle crashes.

To determine whether graduated driver licensing (GDL) systems and nighttime curfews reduce motor vehicle crashes, fatalities, or injuries among young drivers. We used Cochrane Collaboration search strategies to locate studies of graduated licensing or night driving restrictions. Studies were selected if they examined the effects of either (1) a comprehensive graduated driver licensing system including well-integrated components, or (2) nighttime driving restrictions/curfews that could affect young persons' nighttime driving, on a clearly defined crash or injury outcome. Seven studies met inclusion criteria. Two independent studies of the New Zealand graduated licensing program found a sustained 7%-8% reduction in teen driver crash injuries attributable to the program. No other full graduated licensing system has been evaluated to date. Four studies of either a general curfew or a nighttime driving restriction for teens, a key element of graduated licensing, found substantial crash reductions during restricted hours, with 23%-25% lower crash injury and fatality rates for curfews beginning prior to midnight. One study found no change in late night crashes before and after a 1 a.m.-6 a.m. night driving restriction took effect. The logic and empirical bases for graduated licensing are sound. Moreover, there is evidence that one central element, a restriction on nighttime driving by novices, reduces young driver crashes. However, a definitive conclusion about the effectiveness of GDL systems for reducing motor vehicle crashes or crash-related injuries must await examination of other GDL systems. This should be possible within the next few years, as several states and Canadian provinces have recently enacted GDL programs.
Differences in young driver crash involvement in states with varying licensure practices.

Teenage driver licensing practices and the crashes of teenagers were compared in several states with differing laws and policies regarding licensure. High school seniors in Delaware, a state with laws that allow early driving and licensure, reported that they first drove on a public road, obtained a learner's permit and obtained a driver's license at younger ages than high school seniors in other northeastern states (Connecticut, New Jersey and upstate New York). State crash data indicated that Delaware and Connecticut, which allow unrestricted licensing at age 16, showed the highest rates of 16-year-old driver involvements in nonfatal and fatal injury crashes. Pennsylvania and upstate New York, which have night driving curfews for 16 year-olds, showed lower crash rates overall and much lower crash rates during their respective curfew hours. New Jersey and Nassau and Suffolk counties, where unsupervised driving by 16 year-olds is generally not allowed, showed the lowest crash rates for 16 year-olds. Graduated licensing programs that include delayed full-privilege licensure, night driving curfews, and extended periods of supervised practice driving are a possible countermeasure for the high motor vehicle crash rates of young drivers.

The current study examined psychosocial predictors of change in intercourse frequency and number of sexual partners among youth within a socio-ecological framework and assessed whether these determinants vary by stage of adolescent development. Longitudinal data were derived from a large, community study of adolescent risky behavior among predominantly high-risk, African American youth. Significant predictors of intercourse frequency for early adolescents included age, gender, self-worth, and familial factors; for older youth, age, gender, self-worth, curfews, and sense of community exerted significant effects. Among early adolescents, age, gender, self-worth, familial factors, and sense of community predicted change in the number of sexual partners in the previous year, while age, gender, self-worth, parental knowledge, curfews, and sense of community were predictive of change in the number of sexual partners in the previous year among older youth. The current study uses data from the genetic subsample from the National Longitudinal Study of Adolescent Health (Add Health) in waves I and II (ages of 11-19 and 12-20 respectively) to investigate the interaction of the TaqIA polymorphism and poor parental socialization on changes in adolescent marijuana use. Results reveal that TaqIA interacts with poor parental rule setting, but not quality of mother-child communication, to influence changes in marijuana use. Adolescents who are homozygous for the A1 and whose parents allow the youth to set their own curfew experience significant increases in marijuana use during adolescence. In contrast, youths with the A1/A1 genotype whose parents do not allow the adolescent to set their own curfew experience significant decreases in the frequency of marijuana use. These results suggest that direct parental social control may effectively suppress the genetic risk of the A1/A1 genotype on marijuana use in adolescence. The study's limitations are noted.

While gender socialization and practice have been investigated in education, work, and children within the family, there has been minimal research on adolescents and parents. The present research focused on male and female adolescents' perceptions of their mothers' and fathers' behavior in six common family routines: allowances, gifts, use of the family car, curfew, and chores inside and outside the home. Data were gathered from 448 senior high school students, Gender-equal behavior toward sons and daughters was clearly evident in the parental practice of allowances and gift giving, and less evident in permission to use the family car. Traditional behavior was strongly evident in sons' performing outside chores and, to a lesser degree, in daughters' keeping an earlier curfew and doing more inside chores. Fathers were more traditional than mothers in allowance and gift giving to sons, and less traditional than mothers in giving sons permission to use the car. It is evident from the perspective of the teenager, that stereotypical gender roles are being perpetuated in North American home life.

African-American adolescent females' predictors of having sex.

African-American adolescent females are at high risk for HIV infection, acquired primarily as a result of heterosexual intercourse. Multiple regression analyses was used to describe the correlates of the outcome variable, ever having had sex, for low-income African-American adolescent females in a cross-sectional study of 322 (N = 322) mother/daughter pairs. The results revealed that while the daughters' age was positively associated with the outcome variable, the daughters' perceptions of strictness of curfew and maternal monitoring, and the daughters'self-efficacy to refuse sex and their intention to refuse sex were negatively associated with the outcome variable. Mothers tended to report significantly more maternal monitoring and stricter curfews than what their daughters perceived them to be. The daughters'intention to refuse sex mediated the effects of age and self-efficacy to refuse sex on the outcome variable. These results suggest that health-care providers should promote clearer communication between mothers and daughters in order to reduce divergent perceptions and to help mothers facilitate their daughters' self-efficacy and their intention to refuse sex.
Routing of radioactive shipments in networks with time-varying costs and curfews

This research examines routing of radioactive shipments in highway networks with time-dependent travel times and population densities. A time-dependent least-cost path (TDLCP) algorithm that uses a label-correcting approach is adapted to include curfews and waiting at nodes. A method is developed to estimate time-dependent population densities, which are required to estimate risk associated with the use of a particular highway link at a particular time. The TDLCP algorithm is implemented for example networks and used to examine policy questions related to radioactive shipments. It is observed that when only Interstate highway facilities are used to transport these materials, a shipmentmore » must go through many cities and has difficulty avoiding all of them during their rush hour periods. Decreases in risk, increased departure time flexibility, and modest increases in travel times are observed when primary and/or secondary roads are included in the network. Based on the results of the example implementation, the suitability of the TDLCP algorithm for strategic nuclear material and general radioactive material shipments is demonstrated.« less

This study explored low-income and transitional housing environments of women sex workers and their role in shaping agency and power in negotiating safety and sexual risk reduction in Vancouver, Canada. A series of 12 focus group discussions were conducted with 73 women currently involved in street-based sex work. These women were purposively sampled for a range of experiences living in low-income housing environments, including homeless shelters, transitional housing, and co-ed and women-only single room occupancy (SRO) hotels. Drawing on the risk environment framework and theoretical constructs of gender, agency and power, analyses demonstrate that women continue to be vulnerable to violence and sexual and economic exploitation and have reduced ability to negotiate risk reduction resulting from the physical, structural and social environments of current dominant male-centred housing models. Within the physical environment, women described inhabitable housing conditions in SROs with infestations of bedbugs and rats, leading women to even more transitional housing options such as shelters and couch-surfing. In many cases, this resulted in their economic exploitation and increased sexual risk. Within the structural environment, enforcement of curfews and guest policies forced women to accept risky clients to meet curfew, or work outdoors where their ability to negotiate safety and condom use were limited. Certain policies promoted women’s agency and mitigated their ability to reduce risks when selling sex. These included flexible curfews and being able to bring clients home. The social environments of co-ed single-room occupancy hotels resulted in repeated violence by male residents and discrimination by male building staff. Women-only shelters and SROs facilitated ‘enabling environments’ where women developed support systems with other working women that resulted in safer work practices. The narratives expressed in this study reveal the critical need for public health interventions and safer supportive housing to account for the daily lived experiences of women sex workers. Public policy is unsettled with regard to juvenile status offenders--children who are subject to juvenile court jurisdiction for noncriminal behavior such as running away from home, incorrigibility, truancy, and curfew violation. In 1974, the federal Juvenile Justice and Delinquency Prevention Act initiated a national policy of status offender "deinstitutionalization," supporting the development of community-based treatment programs and prohibiting incarceration of these youths. In the following years, most states embraced this policy, sharply reducing status offender detention levels. This article describes factors that have recently worked to erode the federal and state commitment to a policy of status offender deinstitutionalization. These factors include widespread failure to develop adequate services for youths and families; the tragic deaths of runaway youths; rising fear of juvenile crime; and new statehouse majorities promoting agendas of youth discipline and accountability. The article references recent surveys and press reports on the number of runaways, truants, and curfew violators in the United States. In the author's view, policymakers today are torn between their desire to provide services to at-risk youths and families and public pressure to respond to all forms of youth misbehavior with tough new sanctions, including the incarceration of status offenders.

Major airports in the world's air transportation systems face a serious problem in providing greater capacity to meet the ever increasing demands of air travel. This problem could be relieved if airports are allowed to increase their operating time, now restricted by curfews and by relaxing present limits on takeoffs and landings. The key operational issue in extending the present curfews is noise. In response to these increasing restrictive noise regulations, NASA has launched a program to validate through engine testing, noise reduction concepts and technologies that have evolved from the Advanced Subsonic Technologies (AST) Noise Reduction Program. The goal of this AST program was to develop and validate technology that reduces engine noise and improves nacelle suppression effectiveness relative to 1992 technology. Contract NAS3-97144 titled "Engine Validation of Noise Reduction Concepts" (EVNRC) was awarded to P&W on August 12, 1997 to conduct full scale noise reduction tests in two Phases on a PW4098 engine. The following Section 1.2 provides a brief description of the overall program. The remainder of this report provides a detailed documentation of Phase I of the program.

Results—Compared with the pre-GDL group, the restricted licence drivers had fewer crashes at night (p=0.003), fewer involving passengers of all ages (p=0.018), and fewer where alcohol was suspected (p=0.034), but not fewer involving young casualties (p=0.980). Compared with the pre-GDL drivers, those with the full graduated licence had fewer night crashes (p=0.042) but did not differ significantly for any of the other factors examined. Noise from air conditioning and refrigeration condensing units is investigated. The practical aspects of attempting to implement innovative approaches are emphasized. These included: (1) sample selection, (2) noise measurement survey, (3) implementation of aggressive abatement procedures, (4) development and use of a screening graph for determining acceptability of sound rated outdoor unitary equipment, (5) incorporation of noise control considerations, (6) exploration of an operatinal curfew, and (7) development of an incentive/information program.

New teen drivers and their parents: what they know and what they expect.

To assess teens' and parents' knowledge of teen driver safety and to compare teens' and parents' expectations about learning to drive and acquiring a driver's license. A convenience sample of 613 Connecticut teens enrolled in commercial driving schools and one of their parents completed self-administered surveys. Significant differences between teens and parent expectations were noted in practice driving with a parent, driving to approved destinations, unlimited vehicle access, curfew, and teen passenger limits. The discrepancy between teens' and parents' expectations indicates the need to persuade and empower parents to manage their teen's driving experience more rigorously.
Nighttime driving and fatal crash involvement of teenagers.

Data from the 1977 National Personal Transportation Survey and from the Fatal Accident Reporting System were used to compute mileage-based fatal crash involvement rates of drivers, by age, sex, and time of day. Teenagers drive less than older drivers but do more of their driving at night. They have much higher numbers of drivers in fatal crashes based on miles driven than do older drivers; their nighttime rates are particularly high. Sixteen year olds, especially males, have by far the highest fatal crash rates per mile, both nighttime and daytime. More widespread adoption of driving curfew laws would very likely produce substantial reductions in fatalities involving 16 yr old drivers.

Travel markets which would be served by high speed commercial transport aircraft and the ability of the airlines to schedule and route the aircraft in a way that would achieve good daily utilization and productivity are examined. The following areas are considered: (1) identification of the major long-haul city pairs that would most likely demand nonstop service; (2) selection of flight tracks observing alternative sonic boom restrictions; (3) estimation of flight times for all city pairs for the various sonic boom constraints; (4) impact of airport curfews on possible departure and arrival schedules; (5) projection of passenger traffic volumes on the selected city pairs; and (6) potential daily utilization and aircraft productivity.

Adolescents from low-income families face various opportunities and constraints as they develop, with possible ramifications for their well-being. Two contexts of particular importance are the home and the neighborhood. Using adolescent data from the first two waves of the Three City Study (N = 1,169), this study explored associations among housing problems and neighborhood disorder with adolescents’ socioemotional problems, and how these associations varied by parental monitoring and gender. Results of hierarchical linear models suggest that poor quality housing was most predictive of the functioning of girls and of adolescents with restrictive curfews, whereas neighborhood disorder was a stronger predictor for boys. Implications for future research on associations between housing and neighborhood contexts and adolescent development are discussed. PMID:27019574

The effects of ICAO fixed certification limits and local ordinances on acoustic emissions from jets on commercial transport aircraft and costs of operations are explored. The regulations effectively ban some aircraft from operation over populated areas, impose curfews on airports and, in conjunction with local civil aviation rules, levy extra taxes and quotas on noisier equipment. Jet engine manufacturers have attempted to increase the flow laminarity, decrease the exhaust speed and develop acoustic liners for selected duct areas. Retrofits are, however, not usually cost effective due to increased operational costs, e.g., fuel consumption can increase after engine modification because of increased weight. Finally, an attempt is made to assess, monetarily, the costs of noise pollution, wherein fines are levied for noisy aircraft and the money is spent insulating homes from noise.
[Adjustment processes of foreign exchange high school students in Japan].

The main purpose of the present study was to excavate the adjustment problems of 93 high school exchange students in Japan. Questionnaires including Cornell Medical Index (CMI) were administered longitudinally. In addition, individual interviews were held with those who had failed to adjust to the Japanese society. As for the subjective psychosomatic symptoms manifested in CMI, there was no significant sex difference while Asians were successively found to be significantly more liable to diseases and less adjusted than non-Asians. The questionnaires other than CMI disclosed difficulties which exchange students found in adjusting at Japanese home, including delicate personal relationships with host siblings, apparent lack of affective gestures (hugs and kisses), and early curfew. In the meanwhile, language barrier and trifling rules constituted the primary difficulties they faced at host school. On account of prejudice against women, girls had more unpleasant experiences than boys. Through individual interviews, all of the early returners were found to have already had a basic problem in their home countries.
Driving experience, crashes and traffic citations of teenage beginning drivers.

Teenagers were surveyed by telephone every 6 months from their freshman to senior high school years (N=911). Self-reported crash involvements and citations were examined for each teenager's first year of licensure and first 3500 miles driven. Based on survival analysis, the risk of a first crash during the first month of licensure (0.053) was substantially higher than during any of the next 11 months (mean risk per month: 0.025). The likelihood of a first citation during the first month of licensure (0.023) also was higher than during any of the subsequent 11 months (mean risk per month: 0.012). Similarly, when viewed as a function of cumulative miles driven, the risk of a first crash or citation was highest during the first 500 miles driven after licensure. Fewer parental restrictions (e.g. no nighttime curfew) and a lower grade point average (GPA) were associated with a higher crash risk. Male gender, a lower GPA and living in a rural area were associated with a higher citation rate.

The purpose of this study was to identify the problems faced by ambulance drivers working in a conflict zone. This study was conducted on ambulance drivers working for the four major hospitals in Kashmir, India. The drivers were interviewed and asked a series of 30 work-related questions. The individual interviews were conducted over a three-month period in the valley of Kashmir that was affected by continuous violence, strict curfew, and strikes. A total of 35 ambulance drivers were interviewed. Drivers worked an average of 60 h/wk, and they drove an average of 160 km/d. Twenty-nine (83%) of the drivers experienced >1 threat of physical harm; 18 (54%) experienced physical assaults; and 31 (89%) reported evidence of psychological morbidity associated with their jobs. The atmosphere of conflict on the streets of Kashmir impacted the ambulance drivers adversely, both physically as well as mentally. The stress faced by these professionals in conflict zones during their duty hours should be recognized, and corrective measures must be put in place.

According to the last Survey on Drug Use among Secondary School Students (ESTUDES 2014-2015), consumption levels of alcohol and other substances have decreased in the last years in Spain. However, available data on binge drinking remain worrying, given the negative consequences related with this pattern. The aim of this paper is to analyse binge drinking among adolescents, providing updated data on prevalence in addition to information about the consequences and some predictive factors of binge drinking. A correlational method was used for this purpose, comprised of administering a survey to Compulsory Secondary School, High School and Vocational Training students. Based on a sample of 3,419 Galician adolescents aged between 12 and 18 years (M = 14.57; SD = 1.76), the results show that binge drinking is a common and global practice, with few socio-demographic differences but related with a wide range of risk practices. Furthermore, variables such as consumption expectancies, consumption by family and friends, as well as curfew time and allowance money have been identified as interesting predictive factors that should be taken into account at the preventive level.

To study non-fatal unintentional injuries among teenagers and to suggest preventive measures. The emergency care unit of the University Hospital, Umeå, Sweden. All injured teenagers (N = 1044) attending the emergency care unit during 1991 were asked to answer a questionnaire focusing on when, where and how the injury occurred. All available medical records were examined. Data were coded according to the Nordic Medico-Statistical Committees Classification for Accident Monitoring, NOMESCO, and to the Abbreviated Injury Scale, AIS. 1,043 teenagers were treated with sports and transportation related injuries as the most common ones. Most injuries were minor (AIS 1), transportation related injuries had the highest proportion of non-minor injuries (AIS > or = 2), 139 teenagers were admitted for in-patient care. Most injuries occurred during leisure/school time. Sports and transportation related injuries were most frequent. Body weight and length differs among teenagers, we suggest that teenagers should exercise and play together, not only by age, but also to some extent, to height and weight. Curfew laws, a compulsory bicycle helmet law are other injury reducing measures suggested.

Parents raising youth in high-risk communities at times rely on active, involved monitoring strategies in order to increase both knowledge about youth activities and the likelihood that adolescents will abstain from problem behavior. Key monitoring literature suggests that some of these active monitoring strategies predict increases in adolescent problem behavior rather than protect against it. However, this literature has studied racially homogenous, low-risk samples, raising questions about generalizability. With a diverse sample of youth (N = 753; 58% male; 46% Black) and families living in high-risk neighborhoods, bidirectional longitudinal relations were examined among three aspects of monitoring (parental discussions of daily activities, parental curfew rules, and adolescent communication with parents), parental knowledge, and youth delinquency. Parental discussion of daily activities was the strongest predictor of parental knowledge, which negatively predicted delinquency. However, these aspects of monitoring did not predict later delinquency. Findings were consistent across gender and race/urbanicity. Results highlight the importance of active and involved parental monitoring strategies in contexts where they are most needed.

The Athena simulation software supports an analyst from DoD or other federal agency in making stability and reconstruction projections for operational analyses in areas like Iraq or Afghanistan. It encompasses the use of all elements of national power: diplomatic, information, military, and economic (DIME), and anticipates their effects on political, military, economic, social, information, and infrastructure (PMESII) variables in real-world battle space environments. Athena is a stand-alone model that provides analysts with insights into the effectiveness of complex operations by anticipating second-, third-, and higher-order effects. For example, the first-order effect of executing a curfew may be to reduce insurgent activity, but it may also reduce consumer spending and keep workers home as second-order effects. Reduced spending and reduced labor may reduce the gross domestic product (GDP) as a third-order effect. Damage to the economy will have further consequences. The Athena approach has also been considered for application in studies related to climate change and the smart grid. It can be applied to any project where the impacts on the population and their perceptions are important, and where population perception is important to the success of the project.

Drawing on two nationally representative German studies (N(1) = 1744, N(2) = 759), we examined correlates of early, on-time, and late curfew autonomy, a retrospective indicator of behavioural autonomy, in young and middle adulthood (19-37 years of age). Adjustment in four domains was considered: educational attainment, externalizing problem behaviour, subjective well-being, and interpersonal relationships. The early group showed lower adjustment in multiple domains across young and middle adulthood. The late group reported a mixed pattern of adjustment at younger ages (lower externalizing problems, but lower positive affect, lower importance of peers, and lower likelihood to have a partner) and positive adjustment in all domains at older ages. Timing effects were controlled for sociodemographic characteristics and retrospective measures of early adversities, pubertal timing, disclosure to parents, and peer group affiliation in adolescence. Findings show that late behavioural autonomy in its correlates is not simply the opposite of early behavioural autonomy. Bacteria associated with plants have been observed frequently to form assemblages referred to as aggregates, microcolonies, symplasmata, or biofilms on leaves and on root surfaces and within intercellular spaces of plant tissues. In a wide range of habitats, biofilms are purported to be microniches of conditions markedly different from those of the ambient environment and drive microbial cells to effect functions not possible alone or outside of biofilms. This review constructs a portrait of how biofilms associated with leaves, roots and within intercellular spaces influence the ecology of the bacteria they harbor and the relationship of bacteria with plants. We also consider how biofilms may enhance airborne dissemination, ubiquity and diversification of plant-associated bacteria and may influence strategies for biological control of plant disease and for assuring food safety. Motor vehicle crashes are highly elevated among newly licensed teenage drivers. Limits on high-risk driving conditions by driver licensing policies and parents can protect novice teens from negative driving outcomes, while they experience and driving proficiency. The purpose of this research was to evaluate the effects of strict parent-imposed driving limits on driving outcomes during the first year of licensure. A sample of 3,743 Connecticut teens was recruited and randomized to the Checkpoints Program or comparison condition. Assessments conducted at baseline, licensure, 3-, 6-, and 12-months postlicensure included parent-imposed driving limits, traffic violations, and crashes. Bivariate and multivariate analyses were conducted to assess the effects of strict parent limits on traffic violations and crashes during the first year of licensure. Thirty percent of teens reported at least one traffic violation and 40% reported at least one crash. More strict parent-imposed limits at licensure, 3-, 6-, and 12-months postlicensure, were associated with fewer violations and crashes in multivariate analyses. Notably, adherence to recommended night curfew was consistently associated with fewer violations and crashes. The findings indicate that strict parent-imposed limits may protect novice teen drivers from negative driving outcomes.
Reconstruction with different free flaps in oro-facial cancer patients.

In 75 patients following ablative surgery of head and neck cancer, reconstruction was attempted with free tissue transfer techniques under magnification. It was possible to do free tissue transfers in 69 cases. In 6 cases it was not possible to harvest free flaps successfully and alternative reconstructive procedure was carried out due to unavoidable circumstances and various reasons: 1. unsuitable venous drainage, as in Anterior Rib Osteomyocutaneous Composite Flap, AROCF (2 cases), 2. injury to vessels during flap harvest, as in parascapular flap (1 case), 3. residual disease unable to excise (2 cases) and 4. unsuitable proposition (1 case), due to emergency curfew imposed suddenly. These 6 cases were not included in the study. Free tissue transfer was successful in 64 cases (92.7%) and there was a total failure in 5 cases where delayed secondary salvage surgery was performed. Out of 69 cases, in 65 cases reconstructions were carried out immediately, primarily as one-stage operative procedure. Their functional, cosmetic results and complications during the operative and post-operative period are analyzed and discussed. Inter-maxillary fixation was never used to maintain the bite alignment. All cases were given a bite guide prosthesis in the early post-operative period, to improve the bite alignment when it was necessary.

Teenagers, who are members of a longitudinal study of a birth cohort, were interviewed at 15 years of age when the GDLS was first introduced and before they had begun licensure, and again at 18 years of age after they had experience with this licensing system. At both ages the majority (over 70%) agreed with the driving restrictions imposed by this system. After experience with the restrictions, however, significantly more reported being affected a lot by them, than had expected to be at age 15. This was especially true of the restrictions on the carrying of passengers and the night time curfew (10 pm - 5 am). However, few reported that they were affected by the alcohol restriction. Sixty eight per cent of those with a graduated licence reported breaking at least one of the conditions, most frequently carrying passengers. Very few were penalised by the police for this. Generally these young drivers were positively disposed towards the driving restrictions, but noncompliance was common. A full evaluation of all aspects of this licensing system is recommended.
Establishment of a novel tick-Babesia experimental infection model.

Ticks are potent vectors of many deadly human and animal pathogens. Tick-borne babesiosis is a well-recognized malaria-like disease that occurs worldwide and recently has attracted increased attention as an emerging zoonosis. Although the proliferation of Babesia organisms is essential in the vectors, their detailed lifecycle with time information for migration in ticks remains unknown. A novel study model for the elucidation of the migration speed of Babesia parasites in their vector tick, Haemaphysalis longicornis, has been developed using an artificial feeding system with quantitative PCR method. The detectable DNA of Babesia parasites gradually disappeared in the tick midgut at 1 day post engorgement (DPE), and in contrary increased in other organs. The results indicated that the Babesia parasite passed the H. longicornis midgut within 24 hours post engorgement, migrated to the hemolymph, and then proliferated in the organs except the midgut. This time point may be an important curfew for Babesia parasites to migrate in the tick lumen. We also visualized the Babesia parasites in the experimentally infected ticks and in their eggs using IFAT for detecting their cytoskeletal structure, which suggested the successful tick infection and transovarial transmission of the parasite. This model will shed light on the further understanding of tick-Babesia interactions.

Analysis of Otologic Injuries Due to Blast Trauma by Handmade Explosives

Objective The aim of this study is to identify the otologic injuries due to handmade explosive-welded blast travma in the law enforcement officers during the combat operations in the curfew security region and to specify the disorders that Otolaryngology and Head Neck Surgery (OHNS) physicians can face during such operations. Methods Medical records of patients in law enforcement who were initially treated by OHNS physicians of Silopi State Hospital during combat operations, between December 14, 2015 and January 15, 2016 were reviewed. Twenty-five patients with otologic injuries due to blast trauma were included in the study. Trauma characteristics, physical examination findings, and beginning treatments were identified. Results Primary blast injury (PBI) was identified as the major disorder in all 24 cases. Tinnitus and hearing loss were the most frequent complaints. In physical examination, tympanic membrane perforations were found in four ears of three patients. Oral methylprednisolone in decreasing doses for 10 days was commenced as an initial treatment in patients with PBI. Secondary blast injury presented in the form of soft tissue damage in the auricular helix due to shrapnel pieces in one patient and a minor surgery was performed. Conclusion Otologic injuries due to blast trauma may often develop during this type of combat operations. Otologic symptoms should be checked, otoscopic examination should be performed, and patients should consult OHNS physicians as soon as possible after trauma. PMID:29392057

Since the start of the second Palestinian uprising (Al-Aqsa Intifada), and due to the Israeli activities, curfews, closures, and military checkpoints imposed since 2000, the quality of social services rendered by Nablus city has been gradually deteriorating. Solid waste management in Nablus city was badly affected by these conditions, and this situation is negatively affecting health and damaging the environment. Most of these cases were due to reasons beyond the capability of the municipality with its limited resources. This study revealed that some of the important municipal solid waste (MSW) equipment had been damaged during the uprising. The workforce in the MSW system was reduced and certain MSW-related development projects and activities have been frozen due to the current conditions. The city's medical waste incinerator had been phased out and the number of special medical containers had been reduced from 16 to 10. Some MSW compressing trucks had been out of use with no substitute. Another important figure is the number of waste collection workers which decreased from 420 to 301, although the city is growing in premises as well as population. The created unsanitary solid waste transfer station is now a pollution source on its own, causing an ugly scene at the eastern entrance of Nablus city. There should be a comprehensive and urgent solution for this problem and the needed resources should be invested.
An evaluation of Teaching-Family (Achievement Place) group homes for juvenile offenders.

Understanding How Domestic Violence Shelter Rules May Influence Survivor Empowerment.

Domestic violence shelters have historically gone beyond providing emergency residential space for survivors by assisting in obtaining future housing, employment, health care, child care, or legal services. Domestic violence shelters are expected to operate within an empowerment philosophy, with an understanding that survivors are self-determining, can identify their needs, and know what it takes to meet those needs. Recent research has indicated that, as many shelters have become more rigid in creating rules that survivors must follow to access and retain free temporary housing, the result has been survivors' feelings of disempowerment, the complete opposite of what was originally intended. This study builds on the small amount of research conducted regarding survivors' experiences of shelter rules by specifically examining how rules were perceived to affect empowerment. Seventy-three survivors from two domestic violence shelters were asked about their experiences around specific shelter rules relating to curfew, parenting, chores, time limits, food, alcohol, drugs, and medications. A transcendental phenomenological approach was used to analyze the qualitative data, seeking explanations of how survivors made meaning of the rules and how those rules influenced their empowerment. Among those survivors who found the rules problematic, three major themes emerged: (a) rules acted as barriers to carrying out their normal, day-to-day activities; (b) the shelter staff's flexibility with rules was based on contingencies; and (c) rules negatively affected their psychological well-being, and required them to engage in protective behaviors. Recommendations are made for the reexamination and restructuring of rules within domestic violence shelters.

Firearm injuries in the pediatric population: A tale of one city.

Firearm-related injuries are a significant cause of morbidity and mortality in children. To determine current trends and assess avenues for future interventions, we examined the epidemiology and outcome of pediatric firearm injuries managed at our region's two major pediatric trauma centers. Following institutional review board approval, we conducted a 5-year retrospective review of all pediatric firearm victims, 16 years or younger, treated at either of the region's two Level 1 pediatric trauma centers, St. Louis Children's Hospital and Cardinal Glennon Children's Medical Center. There were 398 children treated during a 5-year period (2008-2013) for firearm-related injuries. Of these children, 314 (78.9%) were black. Overall, there were 20 mortalities (5%). Although most (67.6%) patients were between 14 years and 16 years of age, younger victims had a greater morbidity and mortality. The majority of injuries were categorized as assault/intentional (65%) and occurred between 6:00 pm and midnight, outside the curfew hours enforced by the city. Despite a regional decrease in the overall incidence of firearm injuries during the study period, the rate of accidental victims per year remained stable. Most accidental shootings occurred in the home (74.2%) and were self-inflicted (37.9%) or caused by a person known to the victim (40.4%). Despite a relative decrease in intentional firearm-related injuries, a constant rate of accidental shootings suggest an area for further intervention. Prognostic and epidemiologic study, level IV.
Vulnerability re-assessed: the changing face of sex work in Guntur district, Andhra Pradesh.

We conducted a qualitative study to examine the impact of an HIV prevention programme on female sex workers' lives in Guntur district, Andhra Pradesh. The study found evidence that, in addition to the HIV prevention programme, structural and environmental factors had recently changed the way sex work was being practiced. Recent closure of the brothels and implementation of a late-night street curfew by the police meant sex work had become more hidden, with clients often solicited using mobile phones from home or their work place (e.g., in the fields or factories). Sex work had become safer, with violence by non-regular partners rarely reported. Women understood the risks of unprotected vaginal sex and reported using condoms with their clients. However, clients were more frequently requesting anal sex, possibly due to recent exposure to pornography following increased accessibility to modern technologies such as mobile phones and the Internet. Anal sex with clients was common but women were often unaware of the associated risks and reported unprotected anal sex. HIV positive and/or older women faced severe financial hardship and difficulty soliciting sufficient clients, and reported unprotected vaginal and anal sex to earn enough to survive. Taken together, the findings from this study suggest changing vulnerability to HIV in this setting. It will be important for HIV prevention programmes to be flexible and creative in their approaches if they are to continue to reach this target community effectively.

There are a range of multifaceted behavioural and societal factors that combine to contribute to the causes of obesity. However, it is not yet known how particularly countries' cultural norms are contributing to the global obesity epidemic. Despite obesity reaching epidemic proportions in Libya, since the discovery of oil in 1959, there is a lack of information about obesity in Libyan adults. This study sought to explore the views of key informants about the risk and protective factors associated with obesity among Libyan men and women. A series of qualitative semi-structured interviews were conducted with Libyan healthcare professionals and community leaders. Eleven main themes (risk and protective factors) were identified, specifically: socio-demographic and biological factors, socioeconomic status, unhealthy eating behaviours, knowledge about obesity, social-cultural influences, Libya's healthcare facilities, physical activity and the effect of the neighbourhood environment, sedentary behaviour, Libyan food-subsidy policy, and suggestions for preventing and controlling obesity. Key recommendations are that an electronic health information system needs to be implemented and awareness about obesity and its causes and consequences needs to be raised among the public in order to dispel the many myths and misconceptions held by Libyans about obesity. The current political instability within Libya is contributing to a less-active lifestyle for the population due to security concerns and the impact of curfews. Our findings have implications for Libyan health policy and highlight the urgent need for action towards mitigating against the obesity epidemic in Libya.

Since the late 1990s, in England and in Wales, there has been increasing interest in the particular challenges of managing offenders with personality disorder (PD). In 1999, a specialist hostel, managed by the probation service but with a high level of forensic mental health service input, was opened to high-risk PD offenders. To describe the first 93 high-risk residents with PD who were completing sentences under life licence, parole or probation, and their outcome. We investigated the nature of the offences residents had previously committed, their psychological profile in terms of personality patterns on the Millon Clinical Multiaxial Inventory (MCMI-III) and the Psychopathy Checklist-Revised (PCL-R), as well as staff commentary on their progress, to establish whether these factors related to outcome in terms of completion of stay in the hostel or premature discharge. Curfew failures and rearrest rates were also measured. Of the 80 men who completed their residency within the two years of the study, the majority (50) left the hostel for positive reasons under mutual agreement. One-fifth were rearrested while resident, which is a lower rate than would be expected for such a group of offenders. PCL-R scores were predictive of outcome, but so was previous offending history. Self-defeating traits on the MCMI-III and negative comments written by hostel staff were also associated with failure. The hostel development demonstrated that probation and health services can work together to manage violent offenders with high levels of psychological dysfunction, and the evaluation provided some indications of how such arrangements might be enhanced.
A national survey of U.S. juvenile mental health courts.

The authors surveyed U.S. juvenile mental health courts (JMHCs). Forty-one were identified in 15 states, and 34 returned surveys; one was completed on the basis of published information. Topics included the court's history, youths served, inclusion and exclusion criteria, the court process, and services provided. Half (51%) reported that the juvenile court was responsible for the program; for 11% the probation agency had the responsibility, and 17% reported shared responsibility by these entities. Fifty-one percent reported that all youths with any mental disorder diagnosis are eligible. The most commonly reported participant diagnoses are bipolar disorder (27%), depression (23%), and attention-deficit hyperactivity disorder (16%). Seventy percent currently include participants with felony offenses, and 91% with misdemeanors; 67% exclude status offenses, and 21% exclude violent offenses. A guilty plea was required by 63%. Incentives to participate included dismissal of charges (40%), reduction in court hearings (43%), praise by the judge and probation officer (60%), reduction in curfew restrictions (23%), and gift cards or gifts (71%). Sanctions for not participating included increased supervision or hearings (60%), performing community service (54%), and placement in residential detention (60%). Most JMHCs reported use of a multidisciplinary team to coordinate community-based services to prevent protracted justice system involvement. JMHCs are being developed in the absence of systematically collected outcome data. Although they resemble adult mental health courts, they have unique features that are specific to addressing the complex needs of youths with mental disorders involved in the justice system. These include diagnostic and treatment challenges and issues related to involving families and schools.
The protective rate of the feline immunodeficiency virus vaccine: An Australian field study.

A case-control field study was undertaken to determine the level of protection conferred to client-owned cats in Australia against feline immunodeficiency virus (FIV) using a commercial vaccine. 440 cats with outdoor access from five Australian states/territories underwent testing, comprising 139 potential cases (complete course of primary FIV vaccinations and annual boosters for three or more years), and 301 potential controls (age, sex and postcode matched FIV-unvaccinated cats). FIV status was determined using a combination of antibody testing (using point-of-care test kits) and nucleic acid amplification, as well as virus isolation in cases where results were discordant and in all suspected FIV-vaccinated/FIV-infected cats ('vaccine breakthroughs'). Stringent inclusion criteria were applied to both 'cases' and 'controls'; 89 FIV-vaccinated cats and 212 FIV-unvaccinated cats ultimately satisfied the inclusion criteria. Five vaccine breakthroughs (5/89; 6%), and 25 FIV-infected controls (25/212; 12%) were identified, giving a vaccine protective rate of 56% (95% CI -20 to 84). The difference in FIV prevalence rates between the two groups was not significant (P=0.14). Findings from this study raise doubt concerning the efficacy of Fel-O-Vax FIV® under field conditions. Screening for FIV infection may be prudent before annual FIV re-vaccination and for sick FIV-vaccinated cats. Owners should not rely on vaccination alone to protect cats against the risk of acquiring FIV infection; other measures such as cat curfews, the use of 'modular pet parks' or keeping cats exclusively indoors, are recommended. Growing up in a permissive household: what deters at-risk adolescents from heavy drinking?

This study identified psychosocial factors that may deter adolescents living in permissive households from heavy drinking in Grades 9 and 11. Longitudinal data were obtained from 710 youth who completed surveys from Grades 7 to 11. Permissive household was defined based on adolescent reports of whether the parents (1) would be upset if the adolescent drank or used marijuana, (2) knew their child's whereabouts when the adolescent was away from home, and (3) set curfews. Frequency of heavy drinking in the last 30 days was the number of days the adolescent had at least three alcoholic drinks. Three quarters of adolescents from permissive households reported heavy drinking at Grade 9, with less frequent heavy drinking among those who concurrently reported less exposure to peer and adult drinking, less peer approval of drinking, weaker positive beliefs about drinking, a stronger academic orientation, higher resistance self-efficacy, and less delinquency. Further, social influences and alcohol beliefs predicted the frequency of heavy drinking 2 years later among adolescents from permissive households. Although most of these factors were also relevant for adolescents from nonpermissive households, social influences, alcohol beliefs and resistance self-efficacy were stronger predictors of heavy drinking at Grade 9 among youth from permissive households. Growing up in a permissive household was associated with heavy drinking. Nonetheless, several psychosocial factors were associated with less frequent heavy drinking even within this at-risk population. Alcohol prevention programs that target pro-drinking peer and adult influences, positive attitudes toward drinking, and resistance self-efficacy may be particularly important in deterring heavy drinking among adolescents living in permissive households.

Purpose African American youth, particularly those from low-income backgrounds, evidence high rates of negative outcomes associated with three problem behaviors, conduct problems, risky sexual behavior, and substance use. This study used a contextually-tailored version of Problem Behavior Theory (PBT) to examine predictors of the simultaneous development of problem behaviors in this specific cultural group. Methods Socio-contextual and individual variables representing four PBT predictor categories, controls protection, support protection, models risk, and vulnerability risk, were examined as predictors of co-occurring problem behaviors among economically disadvantaged African American adolescents (n = 949). Specifically, the likelihood of following three classes of multiple problem behavior trajectories spanning ages 12 to 18, labeled the “early experimenters,” “increasing high risk-takers,” and “adolescent-limited” classes, as opposed to a “normative” class was examined. Results Among other findings, controls protection in the form of a more stringent household curfew at age 12 was related to a lower likelihood of being in the “early experimenters” and “increasing high risk-takers” classes. Conversely, vulnerability risk manifested as stronger attitudes of violence inevitability was associated with a higher likelihood of being in the “early experimenters” class. However, the PBT category of support protection was not associated with risk trajectory class. More distal neighborhood-level manifestations of PBT categories also did not predict co-occurring behavior problems. Conclusion Guided by an incorporation of contextually-salient processes into PBT, prevention programs aiming to decrease co-occurring problem behaviors among low-income African American adolescents would do well to target both proximal systems and psychological constructs related to perceived security throughout adolescence. Smartphones are prevalently used among adolescents; however, nighttime exposure to blue-enriched light, through electric devices, is known to induce delays of the circadian rhythm phases and poor morning somatic conditions. We therefore investigated whether evening smartphone use may affect sleep-wake cycle and cardiac autonomic nervous system (ANS) activity after awaking in dormitory students. The participants were high school students, living under dormitory rules regarding the curfew, study, meals, lights-out, and wake-up times. The students were forbidden from the use of both television and personal computer in their private rooms, and only the use of a smartphone was permitted. According to prior assessment of smartphone use, we chose age-, sex-, exercise time-matched long (n = 22, >120 min) and short (n = 14, ≤60 min) groups and compared sleep-wake cycle and physiological parameters, such as cardiac ANS activity, blood pressure, and intra-aural temperature. All measurements were performed during 6:30 to 7:00 a.m. in the dormitories. Compared with the short group, the long group showed a significantly lower cardiac ANS activity (2727 ± 308 vs. 4455 ± 667 ms 2 , p = 0.030) with a tendency toward a high heart rate, in addition to later bedtimes during weekdays and more delayed wake-up times over the weekend. Blood pressure and intra-aural temperature did not differ between the groups. In this population, evening smartphone use may be associated with altered sleep-wake cycle and a diminished cardiac ANS activity after awakening could be affecting daytime activities.

The German Aerospace Center (DLR) investigated in the NORAH sleep study the association between a distinct change in nocturnal aircraft noise exposure due to the introduction of a night curfew (11:00 p.m.-5:00 a.m.) at Frankfurt Airport and short-term annoyance reactions of residents in the surrounding community. Exposure-response curves were calculated by random effects logistic regression to evaluate the aircraft noise-related parameters (1) number of overflights and (2) energy equivalent noise level L ASeq for the prediction of short-term annoyance. Data of the NORAH sleep study were compared with the STRAIN sleep study which was conducted by DLR near Cologne-Bonn Airport in 2001/2002 (N = 64), representing a steady-state/low-rate change. The NORAH sleep study was based on questionnaire surveys with 187 residents living in the vicinity of Frankfurt Airport. Noise-induced short-term annoyance and related non-acoustical variables were assessed. Nocturnal aircraft noise exposure was measured inside the residents' home. A statistically significant rise in the portion of annoyed residents with increasing number of overflights was found. Similarly, the portion of annoyed subjects increased with rising L ASeq . Importance of the frequency of fly-overs for the prediction of annoyance reactions was emphasized. The annoyance probability was significantly higher in the NORAH than in the STRAIN sleep study. Results confirm the importance of both acoustical parameters for the prediction of short-term annoyance due to nocturnal aircraft noise. Quantitative annoyance models that were derived at steady-state/low-rate change airports cannot be directly applied to airports that underwent a distinct change in operational and noise exposure patterns.
Challenges of the management of mass casualty: lessons learned from the Jos crisis of 2001.

Jos has witnessed a series of civil crises which have generated mass casualties that the Jos University Teaching Hospital has had to respond to from time to time. We review the challenges that we encountered in the management of the victims of the 2001 crisis. We reviewed the findings of our debriefing sessions following the sectarian crisis of September 2001 and identified the challenges and obstacles experienced during these periods. Communication was a major challenge, both within and outside the hospital. In the field, there was poor field triage and no prehospital care. Transportation and evacuation was hazardous, for both injured patients and medical personnel. This was worsened by the imposition of a curfew on the city and its environs. In the hospital, supplies such as fluids, emergency drugs, sterile dressings and instruments, splints, and other consumables, blood and food were soon exhausted. Record keeping was erratic. Staff began to show signs of physical and mental exhaustion as well as features of anxiety and stress. Tensions rose between different religious groups in the hospital and an attempt was made by rioters to attack the hospital. Patients suffered poor subsequent care following resuscitation and/or surgery and there was neglect of patients on admission prior to the crisis as well as non trauma medical emergencies. Mass casualties from disasters that disrupt organized societal mechanisms for days can pose significant challenges to the best of institutional disaster response plans. In the situation that we experienced, our disaster plan was impractical initially because it failed to factor in such a prolongation of both crisis and response. We recommend that institutional disaster response plans should incorporate provisions for the challenges we have enumerated and factor in peculiarities that would emanate from the need for a prolonged response.
Impact of Georgia's teenage and adult driver responsibility act: 15-year follow-up.

This study was designed to investigate the 15-year impact of Georgia's graduated drivers' licensing (GDL) law, the "Teenage and Adult Driver Responsibility Act" (TADRA), on fatality crashes among young drivers. Fatality Analysis Reporting System (FARS) data for Georgia and Census denominators were used to determine fatal crash rates (FCRs) of drivers ages 16 through 19 who passed through the Georgia GDL system during the 5.5 years prior to through 15.5 years after TADRA's implementation. FCRs of younger drivers were compared to those of drivers ages 20-54 and 55-75 and compared by gender. Attention was given to speeding- and alcohol-related crashes, 2 foci of TADRA. The decline in FCRs has been maintained and even increased through 15.5 years after passage of the law. Extending the curfew and further limiting passengers (passed in 2001) and driver's education and supervised driving requirements (added in 2007) may have contributed. The greatest declines were among 16- and 17-year-olds; most of the gains were among male drivers. The changes were greatest for alcohol- and speeding-related crashes. Those 18 and 19 years old did not demonstrate an increase in FCR over the period studied. Georgia's graduated licensing law, TADRA, has maintained and in some instances increased in effectiveness over the 15.5 years since its inception. Though national research suggests that GDL laws are associated with increased crash rates among 18- to 19-year-old drivers, this has not occurred in Georgia; 18- and 19-year-olds demonstrated no change or reductions in FCR over the 20.5-year period evaluated. Declines were greatest for those driving behaviors targeted by the law.

Legal preparedness is a critical component of comprehensive public health preparedness for public health emergencies. The scope of this study was to assess the usefulness of combining didactic sessions with a tabletop exercise as educational tools in legal preparedness, to assess the impact of the exercise on the participants' level of confidence about the legal preparedness of a public health system, and to identify legal issue areas in need of further improvement. The exercise scenario and the pre- and postexercise evaluation were designed to assess knowledge gained and level of confidence in declaration of emergencies, isolation and quarantine, restrictions (including curfew) on the movement of people, closure of public places, and mass prophylaxis, and to identify legal preparedness areas most in need of further improvement at the system level. Fisher exact test and paired t test were performed to compare pre- and postexercise results. Our analysis shows that a combination of didactic teaching and experiential learning through a tabletop exercise regarding legal preparedness for infectious disease emergencies can be effective in both imparting perceived knowledge to participants and gathering information about sufficiency of authorities and existence of gaps. The exercise provided a valuable forum to judge the adequacy of legal authorities, policies, and procedures for dealing with pandemic influenza at the state and local levels in Massachusetts. In general, participants were more confident about the availability and sufficiency of legal authorities than they were about policies and procedures for implementing them. Participants were also more likely to report the need for improvement in authorities, policies, and procedures in the private sector and at the local level than at the state level.
Graduated driver licensing decal law: effect on young probationary drivers.

Decal laws have been implemented internationally to facilitate police enforcement of graduated driver licensing (GDL) restrictions (e.g., passenger limit, nighttime curfew) but have not been evaluated. New Jersey implemented the first decal law in the U.S. on May 1, 2010. The aim of this study was to evaluate the effect of New Jersey's law on the rate of citations issued for violation of GDL restrictions and police-reported crashes among probationary drivers aged <21 years and to estimate the number of probationary drivers whose crashes were prevented by the law. New Jersey's licensing and crash databases were linked from January 1, 2008 to May 31, 2011, and each driver's license status, age, and outcome status were ascertained for each month. Monthly rates were calculated as the proportion of probationary drivers who experienced the outcome in that month. The pre-law period was defined as January 2008-January 2010 and the post-law period as May 2010-May 2011. Negative binomial regression models with robust SEs were used to determine the law's effect on crash and citation rates (adjusted for gender, seasonal trends, and overall trends) and estimate prevented crashes. Analyses were conducted in 2012. In the first year post-law, there was a 14% increase in the GDL citation rate (adjusted rate ratio 1.14 [95% CI=1.05, 1.24]); a 9% reduction in the police-reported crash rate (adjusted rate ratio 0.91 [95% CI=0.86, 0.97]), and an estimated 1624 young probationary drivers for whom a crash was prevented. Findings suggest that the law is positively affecting probationary drivers' safety. Results contribute to building the evidence base for the effectiveness of decal laws and provide valuable information to U.S. and international policymakers who are considering adding decal laws to enhance existing GDL laws. Challenges of the management of mass casualty: lessons learned from the Jos crisis of 2001

Background Jos has witnessed a series of civil crises which have generated mass casualties that the Jos University Teaching Hospital has had to respond to from time to time. We review the challenges that we encountered in the management of the victims of the 2001 crisis. Methodology We reviewed the findings of our debriefing sessions following the sectarian crisis of September 2001 and identified the challenges and obstacles experienced during these periods. Results Communication was a major challenge, both within and outside the hospital. In the field, there was poor field triage and no prehospital care. Transportation and evacuation was hazardous, for both injured patients and medical personnel. This was worsened by the imposition of a curfew on the city and its environs. In the hospital, supplies such as fluids, emergency drugs, sterile dressings and instruments, splints, and other consumables, blood and food were soon exhausted. Record keeping was erratic. Staff began to show signs of physical and mental exhaustion as well as features of anxiety and stress. Tensions rose between different religious groups in the hospital and an attempt was made by rioters to attack the hospital. Patients suffered poor subsequent care following resuscitation and/or surgery and there was neglect of patients on admission prior to the crisis as well as non trauma medical emergencies. Conclusion Mass casualties from disasters that disrupt organized societal mechanisms for days can pose significant challenges to the best of institutional disaster response plans. In the situation that we experienced, our disaster plan was impractical initially because it failed to factor in such a prolongation of both crisis and response. We recommend that institutional disaster response plans should incorporate provisions for the challenges we have enumerated and factor in peculiarities that would emanate from the need for a prolonged response. Scott: an 11-year-old boy with repetitive lying.

Scott, an 11-year-old boy in the fifth grade, is brought to his pediatrician, Dr. Lewis, by his maternal grandparents with the principle concern that "he lies constantly." Scott lived with his maternal grandparents since he was 2 years old, and they have full custody. His mother and father had serious substance abuse problems. The grandparents provide a stable home for Scott and his 15-year-old sister. Scott has had no contact with his mother in more than 6 years and sees his father infrequently. During the last visit with his father, he was so inebriated that he was thrown out of the movie theatre and barely avoided several car accidents on the way home. He left the children at the curb of their home and made them promise that they would lie to their grandparents about the reasons for the early return. Scott was diagnosed with attention-deficit hyperactivity disorder (ADHD) in second grade. Methylphenidate (36 mg) provides improvement in attention and concentration. His grandfather describes Scott as highly unpredictable. When he is the "good Jake," he is eager to help, polite, and caring. When Scott gets behind in school or is avoiding his chores and assignments, he lies by saying that he got it all done, even though he knows his grandfather will discover the lie and punish him. When confronted with reports from school, Scott often lies and may develop more elaborate confabulatory stories. His grandfather admits that he becomes irate at these moments. He responds by removing Scott's privileges. When he planned to take Scott to see his favorite sport team in the playoffs, Scott was caught in a lie the day of his departure. His grandfather offered him a chance to fess up, pay a small price in extra chores, and save the trip. Scott stubbornly refused to admit that he lied and lost the trip. His grandfather worries that Scott has no "moral compass." He takes things that do not belong to him and violates household curfew rules. He has never been physically aggressive or has never stole items from a store. He takes his sister's CD player or his grandfather's cell phone even when he has been told not to. He will then lie that he did not take it. Even when it is pulled out of his backpack, he will say he did not put it in there. His grandfather is a businessman with high moral integrity. He loves his grandson and is eager to help him. He asks Dr. Lewis what they should do about Scott's persistent lying.
Graduated driver licensing for reducing motor vehicle crashes among young drivers.

Graduated driver licensing (GDL) has been proposed as a means of reducing crash rates among novice drivers by gradually introducing them to higher risk driving situations. To examine the effectiveness of GDL systems in reducing crash rates of young drivers. Studies were identified through searches of MEDLINE, EMBASE, CINAHL, Healthstar, Web of Science, NTIS Bibliographic Database, TRIS Online, SIGLE, the World Wide Web, relevant conference proceedings, consultation with experts and authors, and reference lists. The search was not restricted by language or publication status. Studies were included if: 1) they compared outcomes pre- and post-implementation of a GDL program within the same jurisdiction, 2) comparisons were made between jurisdictions with and without GDL, or 3) both. Studies had to report at least one objective, quantified outcome. Two reviewers independently screened searches and assessed the full text of potentially relevant studies for inclusion using a standard form. Data were extracted by one reviewer and checked by a second. Additional data were requested from authors. Results were not pooled due to substantial heterogeneity between studies. Percentage change was calculated for each year after the intervention, using one year prior to the intervention as the baseline rate. Results were adjusted by internal controls. Analyses were stratified by different denominators (population, licensed drivers). Results were calculated for the different crash types (overall, injury, fatal, night-time, alcohol, and those resulting in hospitalization). Results were presented for 16 year-olds alone and all teenage drivers combined. We included 13 studies evaluating 12 GDL programs that were implemented between 1979 and 1998 in the US (n=7), Canada (3), New Zealand (1), and Australia (1). Programs varied in their restrictions during the intermediate stage: e.g. night curfews (8); limitations of extra passengers (2); roadway restrictions (1). Based on the Insurance Institute for Highway Safety classification scheme, no programs were good, six were acceptable, five were marginal, and one was poor. Reductions in crash rates were seen in all jurisdictions and for all crash types. Among 16 year-old drivers, the median decrease in per population overall crash rates during the first year was 31% (range 26-41%). Per population injury crash rates were similar (median 28%, range 4-43%). Results for all teenage drivers, rates per licensed driver, and rates adjusting for internal controls were generally reduced when comparing within jurisdictions. The existing evidence shows that GDL is effective in reducing the crash rates of young drivers, although the magnitude of the effect is unclear. The conclusions are supported by consistent direction of the findings, and the temporal relationship and plausibility of the association. The reviewers have made recommendations for primary research on GDL (e.g. study methods, standardized reporting of outcomes and results, long-term follow-up). The project has also shown what is needed to carry out systematic reviews of observational studies (e.g. quality assessment instruments).
