Ce present projet de recherche a permis. de creer un modele par elements finis du muscle strie humain dans le but d'etudier les mecanismes engendrant les lesions musculaires traumatiques. Ce modele constitue une plate-forme numerique capable de discerner l'influence des proprietes mecaniques des fascias et de la cellule musculaire sur le comportement dynamique du muscle lors d'une contraction excentrique, notamment le module de Young et le module de cisaillement de la couche de tissu conjonctif, l'orientation des fibres de collagene de cette membrane et le coefficient de poisson du muscle. La caracterisation experimentale in vitro de ces parametres pour des vitesses de deformation elevees a partir de muscles stries humains actifs est essentielle pour l'etude de lesions musculaires traumatiques. Le modele numerique developpe est capable de modeliser la contraction musculaire comme une transition de phase de la cellule musculaire par un changement de raideur et de volume a l'aide des lois de comportement de materiau predefinies dans le logiciel LS-DYNA (v971, Livermore Software Technology Corporation, Livermore, CA, USA). Le present projet de recherche introduit donc un phenomene physiologique qui pourrait expliquer des blessures musculaires courantes (crampes, courbatures, claquages, etc.), mais aussi des maladies ou desordres touchant le tissu conjonctif comme les collagenoses et la dystrophie musculaire. La predominance de blessures musculaires lors de contractions excentriques est egalement exposee. The paper provides materials to make up risk groups from decreed contingents by using the database developed and introduced in the Novomoskovsk district, as well as a programme for rapid determination of the risk of tuberculosis. This procedure reduces a scope of fluorographic surveys among the decreed contingents, as well as their expenditures by 60%. Moreover, it may be useful for professional choice in the employment of the decreed persons, which may promote a decrease in the incidence of tuberculosis among them.

Lors de l'etude structurelle d'une aile d'avion, il est difficile de modeliser fidelement les forces aerodynamiques subies par l'aile de l'avion. Pour faciliter l'analyse, on repartit la portance maximale theorique de l'aile sur son longeron principal ou sur ses nervures. La repartition utilisee implique que l'aile entiere sera plus resistante que necessaire et donc que la structure ne sera pas totalement optimisee. Pour pallier ce probleme, il faudrait s'assurer d'appliquer une repartition aerodynamique de la portance sur la surface complete de l'aile. On serait donc en mesure d'obtenir une repartition des charges sur l'aile beaucoup plus fiable. Pour le realiser, nous aurons besoin de coupler les resultats d'un logiciel calculant les charges aerodynamiques de l'aile avec les resultats d'un logiciel permettant sa conception et son analyse structurelle. Dans ce projet, le logiciel utilise pour calculer les coefficients de pression sur l'aile est XFLR5 et le logiciel permettant la conception et l'analyse structurelle sera CATIA V5. Le logiciel XFLR5 permet une analyse rapide d'une aile en se basant sur l'analyse de ses profils. Ce logiciel calcule les performances des profils de la meme maniere que XFOIL et permet de choisir parmi trois methodes de calcul pour obtenir les performances de l'aile : Lifting Line Theory (LLT), Vortex Lattice Method (VLM) et 3D Panels. Dans notre methodologie, nous utilisons la methode de calcul 3D Panels dont la validite a ete testee en soufflerie pour confirmer les calculs sur XFLR5. En ce qui concerne la conception et l'analyse par des elements finis de la structure, le logiciel CATIA V5 est couramment utilise dans le domaine aerospatial. CATIA V5 permet une automatisation des etapes de conception de l'aile. Ainsi, dans ce memoire, nous allons decrire la methodologie permettant l'etude aerostructurelle d'une aile d'avion.

Ces essais ont ete effectues sur le simulateur de vol concu et fabrique par CAE Inc. qui possede le niveau D de la dynamique de vol. Une methodologie basee sur les reseaux de neurones optimises a l'aide d'un algorithme intitule le "grand deluge etendu" est utilisee dans la conception de ce systeme d'identification. Plusieurs tests de vol pour differentes altitudes et differents nombres de Mach ont ete realises afin de s'en servir comme bases de donnees pour l'apprentissage des reseaux de neurones. La validation de ce modele a ete realisee a l'aide des donnees du simulateur. Malgre la nonlinearite et la complexite du systeme, les parametres du moteur ont ete tres bien predits pour une enveloppe de vol determinee. Ce modele estime pourrait etre utilise pour des analyses de fonctionnement du moteur et pourrait assurer le controle de l'avion pendant cette phase de croisiere. L'identification des parametres du moteur pourrait etre realisee aussi pour les autres phases de montee et de descente afin d'obtenir son modele complet pour toute l'enveloppe du vol de l'avion Cessna Citation X (montee, croisiere, descente). Cette methode employee dans ce travail pourrait aussi etre efficace pour realiser un modele pour l'identification des coefficients aerodynamiques du meme avion a partir toujours des essais en vol. This Decree-Law rationalizes and updates maternity and paternity leave regulations introduced in Portuguese Law No. 4/84 of 5 April 1984 and Decree-Law No. 136/85 of 3 May 1985. The following are the major changes made by the Decree-Law: 1) the establishment of a six-month waiting period as an eligibility requirement, irrespective of the professional category of the recipient; 2) retroactive computation of the eligibility period from the time of the initial payment of the social security tax associated with other welfare benefits; 3) increase in the amount of the benefit for children requiring parental assistance during an illness to 65% of an index expressing a proportion of past contributions by individual recipients; 4) establishment of minimum benefits to be granted to casual workers or to workers with low incomes. These minimum benefits equal 50% of the benefit for which full-time workers in any given occupational category are eligible. In addition, the Decree-Law provides that workers on leave of absence with pay and current recipients of unemployment benefits are not eligible to receive maternity and paternity benefits dealt with by the Decree-Law. full text

Avoiding "A Massive Spin-Off Effect in West Africa and Beyond": The Tobacco Industry Stymies Tobacco Control in Nigeria.

Nigeria plays important economic and political roles in Africa and is a significant market for the tobacco industry. This study describes the tobacco industry's efforts to block Nigeria's early tobacco control attempts, especially the Tobacco Smoking (Control) Decree 20 of 1990, and efforts to strengthen the Decree in 1995. Analysis of documents from the Truth Tobacco Documents Library and other Internet resources related to Nigeria's Decree 20 and earlier tobacco control efforts. The World Conferences on Smoking and Health and World Health Organization in the late 1970s spurred the Nigerian government to take steps towards tobacco regulation. In response, the tobacco industry lobbied government ministries, used front groups and its trade group, the Tobacco Advisory Council of Nigeria, to block and weaken government efforts. The industry obtained a draft of Decree 20 two years before it was enacted, considered the Decree anti-business and proposed language that led to the passage of a weaker Decree in 1990. It also attempted to influence a potential review of the Decree in 1995. Decree 20 was a strong law for its time, but was weakened due to tobacco industry interference. Nigeria ratified the WHO Framework Convention on Tobacco Control (FCTC) in 2005, and enacted a comprehensive National Tobacco Control Act (NTCA) in May 2015. Lessons learned from Decree 20's experience should be applied to protect NTCA 2015, and in compliance with WHO FCTC Article 5.3 which require parties to protect tobacco control policies from tobacco industry interference. This is the first detailed account of tobacco industry interference with tobacco legislation in Africa. The emergence of tobacco control in Nigeria threatened the tobacco industry, which believed that success in Nigeria would have a "domino effect" in Africa. The industry used lobbying and front groups to successfully block and weaken Nigeria's tobacco control, especially the Tobacco Smoking (Control) Decree 20 of 1990 and efforts to strengthen it in 1995. Nigeria and other African countries must learn from this history to protect tobacco control policies from the tobacco industry's vested interests and vigorously implement Article 5.3 of the WHO FCTC. © The Author 2017. In 1991, an article appeared proposing a plan for "Responsible National Health Insurance" (RHI) that contained three crucial elements supported by economists affiliated with two conservative policy institutions (the American Enterprise Institute and the Heritage Foundation). The central purpose of this article is to revisit RHI in light of developments over the past decade, and to make the case that liberals, rather than reject RHI, now should support it, provided conservatives agree to sufficient funding. FROM A CONTRACTUAL LOGIC TO A PROBABILISTIC APPROACH: Since 1936, reflections on the relationship between the physician and the patient have progressed within the context of a contractual legal concept. Its contents have been based more on jurisprudence than on the physicians' reflections with regard to their practice. Associated with this contractual logic, some confusion exists between a lesion, a medical concept, and a handicap, which is the social consequence of one's status of health. This has been reinforced by the scientific progress made in medicine, which privileges a probabilistic approach based on scientific data, rather than the uncertainty, inscribed in the dialogue and singularity of the encounter. REGARDING THE PERRUCHE DECREE: We analyzed the reports submitted to the court of cessation regarding the Perruche affair, together with the first chapter of the law concerning the rights of the patients and the quality of health system, by studying the extent to which this decision was inscribed or not in the continuity of past legal decisions, and whether the characteristics described above were present. In this decree, the predominating contractual logic is that the notion of handicap is not analyzed, and the scientific and probabilistic conception of medical practice is involved in the debate. THE NECESSITY FOR REFLECTION: Our discussion insists on the necessity of constructing a reflection on the medical presentation; society cannot accept that this be reduced to the strict respect of the elements of the contract, defined by legal decisions. It should be carefully thought out by health professionals, and in collaboration with human science.

This Decree regulates the establishment of indigenous reservations in Colombia. Under the Decree, an indigenous reservation is a legal institution constituted by the transfer to an indigenous community of title to the property where the community is situated or where it will be located. The property may be cultivated or uncultivated. The Decree sets forth the requirements for qualifying for the transfer of such land. Before the land is transferred, the Colombian Institute for Agricultural Reform is to monitor and review the proposed reservation. It is to undertake a socioeconomic and legal study of the indigenous community and to ascertain the community's origins, institutions, customs, and property claims. This Colombian Decree authorizes civil divorce to take place before a notary by means of public document when the spouses are in mutual agreement; the ground for the divorce is separation, either decreed by a court or formalized before a notary; and the separation has lasted more than 2 years. Such a divorce produces the same legal effects as a divorce decreed by a court. The document is to set forth the duties of both spouses with respect to the provisions of sentence 3 of Article 166 of the Civil Code. If there are minors, the document must receive the approval of the municipal or district representative. Decree No. 2275 of 7 October 1989 (Diario Oficial, 7 October 1989, pp. 75-76) amends this Decree to provide that the judgment of the municipal or district representative must be made within 10 days after the document is received. Sentence 3 of Article 166 of the Civil Code requires that spouses who are separating by mutual consent send to a judge their agreement with respect to the care and support of children, which the judge may reject if necessary in the interests of the children.

Les resultats de nouveaux calculs detailles de levitation radiative dans des naines blanches chaudes utilisant la base de donnees atomiques TOPBASE sont presentes. Des accelerations radiatives et des abondances d'equilibre ont ete calculees pour C, N, O, Ne, Na, Mg, Al, Si, S, Ar, Ca, et Fe, sur des grilles de modeles d'enveloppes stellaires riches en hydrogene et riches en helium. La grille des modeles de DA a des gravites telles que log g = 7.0, 7.5, 8.0, et 8.5, et couvre les temperatures effectives pour lesquelles 100,000K >= T_ {rm eff} >= 20,000K par sauts de 2,500K. La grille des modeles de DO/DB est similaire mais se prolonge a T_{rm eff } = 130,000K. Les resultats d'abondances d'equilibre de Ni dans des DA utilisant la base de donnees de Kurucz sont aussi presentes. Nous discutons dans les moindres details de la physique incluse dans les calculs afin de fournir une bonne comprehension physique de la levitation radiative sous les conditions rencontrees dans les naines blanches. Nous discutons aussi de la forme et de la dependance en fonction de la profondeur des reservoirs des elements, crees par un equilibre entre l'acceleration radiative et la gravite locale effective dans differentes enveloppes stellaires. Nous soulignons le role important joue dans la morphologie de ces reserviors par les etats d'ionisation dominants se trouvant dans la configuration gaz noble. Nos resultats centraux sont presentes sous forme de figures montrant le comportement de l'abondance photospherique estimee pour chaque element en fonction de la temperature effective et de la gravite de surface. Nous apportons egalement des ameliorations aux calculs de levitation radiative en utilisant une approche de modeles d'atmospheres pour les etoiles riches en hydrogene. Nous mettons l'emphase, en particulier, sur le role des traces d'elements lourds qui peuvent etre presents dans le plasma. Nous utilisons une table d'opacite monochromatique detaillee calculee pour un plasma compose de H contenant de faibles quantites de C, N, O, et Fe pour illustrer comment les abondances d'equilibre des elements supportes reagissent a la redistribution du flux causee par l'addition de ces traces de matiere opaque. Nous traitons ce probleme, en premier lieu, selon une approche basee sur des modeles d'enveloppes et, en second lieu, selon une approche basee sur des modeles d'atmospheres. Nous considerons aussi deux autres ameliorations concernant, premierement, un traitement plus sophistique de la redistribution de la quantite de mouvement d'un ion suite a l'absorption d'un photon, et deuxiemement, l'utilisation d'une meilleure valeur de la largeur du profil de raie associee a l'elargissement par pression. Bien qu'une poignee d'abondances soient disponibles a partir de l'analyse des observations qui ont ete faites jusqu'a maintenant, nous sommes neanmoins capables de conclure, a l'aide de comparaisons detaillees, que la theorie de la levitation radiative a l'equilibre ne reussit pas a expliquer quantitativement les patrons d'abondances des elements lourds observes dans les naines blanches chaudes. Au moins un autre mecanisme doit competionner avec la levitation radiative et le triage gravitationnel dans les atmospheres/enveloppes des naines blanches chaudes. Nous suggerons la possibilite d'un vent stellaire ou encore la possibilite d'accretion.

Cette recherche vise a la mise en oeuvre optique de reseaux neuronaux. Deux architectures differentes sont proposees. La premiere est la memoire associative permettant d'associer a un objet quelconque une sortie arbitraire tout en preservant l'information sur sa position. La seconde architecture, le classificateur neuronal pour le controle robotique, permet l'identification d'une entree et son classement selon differentes categories. La sortie est compatible avec les systemes numeriques standard. Pour realiser ces architectures, une approche modulaire est privilegiee. Le correlateur constitue le module de base des realisations. Differents modules sont de plus introduits pour realiser convenablement les operations neuronales. Le premier de ces modules est le seuil optoelectronique permettant de realiser une fonction non lineaire, element essentiel des reseaux neuronaux. Le second module a etre introduit est l'encodeur optonumerique, utile au classement des objets. Le probleme de l'enregistrement de la memoire est aborde a l'aide du codage iteratif global.

This Decree-Law establishes criteria for the determination of final compensation for expropriation or nationalization of land or agricultural capital. The lack of legal definition of criteria for the determination of such final compensation was a gap in recent Portuguese legislation on land reform. Although the holdings of many landlords and farmers had been nationalized or expropriated after 11 March 1975, the compensation obtained prior to the enactment of this Decree-Law was provisional and reflected outdated values. This Decree-Law closes this gap by establishing the criteria for the determination of final compensation with adjusted value. Under it, any farmer whose land or capital has been nationalized or expropriated since March 1975 may apply for final compensation, from which shall be deducted the value of any provisional compensation previously obtained and any property previously devolved as a "right of reserve" pursuant to pertinent legislation (see Law No. 109/88 of 26 September 1988 and Decree No. 44/88 of 14 December 1988). The final compensation shall be determined by administrative procedure in which the Ministry of Agriculture, Fisheries, and Food; the Ministry of Finance; and the applicant are represented. full text

All hydrological processes are affected by the spatial variability of the physical parameters of the watershed, and also by human intervention on the landscape. The water outflow from a watershed strictly depends on the spatial and temporal variabilities of the physical parameters of the watershed. Ce travail consiste en la simulation numerique des ecoulements internes compressibles bidimensionnels laminaires et turbulents. On s'interesse, particulierement, aux ecoulements dans les ejecteurs supersoniques. Les equations de Navier-Stokes sont formulees sous forme conservative et utilisent, comme variables independantes, les variables dites enthalpiques a savoir: la pression statique, la quantite de mouvement et l'enthalpie totale specifique. Une formulation variationnelle stable des equations de Navier-Stokes est utilisee. Elle est base sur la methode SUPG (Streamline Upwinding Petrov Galerkin) et utilise un operateur de capture des forts gradients. Un modele de turbulence, pour la simulation des ecoulements dans les ejecteurs, est mis au point. Il consiste a separer deux regions distinctes: une region proche de la paroi solide, ou le modele de Baldwin et Lomax est utilise et l'autre, loin de la paroi, ou une formulation nouvelle, basee sur le modele de Schlichting pour les jets, est proposee. Une technique de calcul de la viscosite turbulente, sur un maillage non structure, est implementee. La discretisation dans l'espace de la forme variationnelle est faite a l'aide de la methode des elements finis en utilisant une approximation mixte: quadratique pour les composantes de la quantite de mouvement et de la vitesse et lineaire pour le reste des variables. La discretisation temporelle est effectuee par une methode de differences finies en utilisant le schema d'Euler implicite. Le systeme matriciel, resultant de la discretisation spatio-temporelle, est resolu a l'aide de l'algorithme GMRES en utilisant un preconditionneur diagonal. Les validations numeriques ont ete menees sur plusieurs types de tuyeres et ejecteurs. La principale validation consiste en la simulation de l'ecoulement dans l'ejecteur teste au centre de recherche NASA Lewis. Les resultats obtenus sont tres comparables avec ceux des travaux anterieurs et sont nettement superieurs concernant les ecoulements turbulents dans les ejecteurs.

This Decree provides that foreigners are not to be permitted to work in Egypt until they have obtained a work permit from the Office of Manpower and Training. In deciding whether to grant a work permit, the Office is to consider the following principles: 1) a foreigner should not be competing with Egyptian citizens who are equally or better qualified; 2) a foreigner must comply with regulations relating to skills and experience established by Egyptian law; and 3) a foreigner born and permanently resident in Egypt is to be given priority over other foreigners. The Office is also to consider the need of the Egyptian economy and of the specific place of work for the foreigner's work. The total number of foreigners employed is not to exceed 10% of the work force of any establishment, although exceptions can be made to this rule in the interests of the Egyptian economy. Exceptions to the provisions of this Decree can also be made for foreigners married to Egyptian citizens. In addition, persons of indeterminate nationality residing in Egypt at least 15 years and political refugees certified as such are to be given priority in employment. Under the Decree, employers are to keep a register of foreigners employed. C'est dans cette optique qu'est ne le projet MDO-505 intitule Morphing Architectures and Related Technologies for Wing Efficiency Improvement. L'objectif de ce projet vise a concevoir une aile adaptative active servant a ameliorer sa laminarite et ainsi reduire la consommation de carburant et les emissions de l'avion. Les travaux de recherche realises ont permis de concevoir et optimiser une peau en composite adaptative permettant d'assurer l'amelioration de la laminarite tout en conservant son integrite structurale. D'abord, une methode d'optimisation en trois etapes fut developpee avec pour objectif de minimiser la masse de la peau en composite en assurant qu'elle s'adapte par un controle actif de la surface deformable aux profils aerodynamiques desires. Le processus d'optimisation incluait egalement des contraintes de resistance, de stabilite et de rigidite de la peau en composite. Suite a l'optimisation, la peau optimisee fut simplifiee afin de faciliter la fabrication et de respecter les regles de conception de Bombardier Aeronautique. Ce processus d'optimisation a permis de concevoir une peau en composite dont les deviations ou erreurs des formes obtenues etaient grandement reduites afin de repondre au mieux aux profils aerodynamiques optimises. Les analyses aerodynamiques realisees a partir de ces formes ont predit de bonnes ameliorations de la laminarite. Par la suite, une serie de validations analytiques fut realisee afin de valider l'integrite structurale de la peau en composite suivant les methodes generalement utilisees par Bombardier Aeronautique. D'abord, une analyse comparative par elements finis a permis de valider une rigidite equivalente de l'aile adaptative a la section d'aile d'origine. Le modele par elements finis fut par la suite mis en boucle avec des feuilles de calcul afin de valider la stabilite et la resistance de la peau en composite pour les cas de chargement aerodynamique reels. En dernier lieu, une analyse de joints boulonnes fut realisee en utilisant un outil interne nomme LJ 85 BJSFM GO.v9 developpe par Bombardier Aeronautique. Ces analyses ont permis de valider numeriquement l'integrite structurale de la peau de composite pour des chargements et des admissibles de materiaux aeronautiques typiques.

A Supreme Decree emitted by the government of Bolivia instituted the celebration of the June solstices in view of the fact that the indigenous people, both the Andean highlands and the Amazon and Chaco, "have commemorated this event for thousands of years" (Gobierno del Estado Plurinacional de Bolivia, Decreto Supremo N° 0173, June16, 2009, La Paz). In the case of the lowlands' indigenous, particularly the Guarani people, the decree mentions the planet Venus as the argument for this celebration. In this case of study and in light of astronomical and ethnographic evidence, we analyze the relevance of this decree in the case of the Guarani people of the Bolivian Chaco region, known as "Chiriguanos".

A Decree of the Supreme Court of the United States in 1954 established the position of Delaware River Master. The Decree authorizes diversions of water from the Delaware River Basin (Figure 1) and requires compensating releases from certain reservoirs of the City of New York to be made under the supervision and direction of the River Master. Reports to the Court, not less frequently than annually, were stipulated. During the 1986 report year, December 1, 1985, to November 30, 1986, precipitation and runoff varied from below average to above average in the Delaware River Basin. For the year as a whole, precipitation was 4.3 inches above average. Runoff was near average. Operations were under a status of drought at the beginning of the report year. The drought emergency was terminated on December 18, 1985, by the Delaware River Basin Commission, and operations were returned to normal as prescribed by the Decree for the remainder of the report yr. Storage in the reservoirs increased to capacity during the winter months and all New York City Delaware River Basin reservoirs spilled throughout the year. Diversions from Delaware River Basin by New York City and New Jersey did not exceed those authorized by the terms of the Amended Decree. Releases were made as directed by the River Master at rates designed to meet the Montague flow objective on 69 days during the June to November period. Releases were made at conservation rates or at rates designed to relieve thermal stress in the streams downstream from the reservoirs at other times. The excess release quantity as defined by the Decree was not expended by end of the report year. In 1998, the government of Mali adopted a national pharmaceutical policy aimed at promoting a supply system for generic essential medicines that would guarantee equal access for all citizens. Distribution and delivery is a shared responsibility of both public and private sectors (wholesalers and pharmacies). To influence private sector behaviour, the national policy uses a combination of government regulation and market forces. In 2006, the government issued a decree fixing maximum prices in the private sector for 107 prescription drugs from the national list of 426 essential medicines. The current study assessed the impact of this intervention on the evolution of market prices (wholesale and retail), and the subsequent availability and public access to essential medicines in Mali. A cross-sectional descriptive survey was conducted in February and May 2006, and January 2009, with 16 wholesalers and 30 private drugstores in Bamako, Mali. The overall availability of essential medicines at private wholesalers (p=1) and pharmacies (p=0.53) was identical before and after the enforcement of the 2006 decree fixing maximum drug prices. Contrary to concerns expressed by wholesalers and pharmacies, and the other stakeholders, the decree did not impact negatively on availability of essential medicines. In fact, median wholesale prices in 2009 were 25.6% less than those fixed by the decree. In private pharmacies, retail prices were only 3% more expensive than the recommended prices, compared with being 25.5% more expensive prior to enforcement of the decree. The study shows that prices of essential medicines in Mali have evolved favourably towards the prices recommended by the government decree. Further, the study contributes to mounting evidence that market regulation by governments does not necessarily negatively affect drug availability; in fact, given the reduction in prices, the study shows that Malians arguably have better access to more affordable essential medicines. The provisions contained in the Legislative Decree no.26 signed by Italy's President on 4th March 2014 will have a considerable impact on the future of experiments with animals. The article briefly describes the stages of the decree's complicated passage through Parliament and the resulting text, which includes bans on: the use of animals for xenotransplants or research on drugs of abuse; the breeding of dogs, cats and non-human primates for experimental use; research without anaesthetic or analgesics that causes pain to the animal, except when anaesthetics or analgesics are being investigated. There is widespread feeling in the scientific community that these provisions will hinder the advancement of biomedical research in Italy.

La caracterisation des proprietes mecaniques de l'os cortical est un domaine d'interet pour la recherche orthopedique. En effet, cette caracterisation peut apporter des informations primordiales pour determiner le risque de fracture, la presence de microfractures ou encore depister l'osteoporose. Les deux principales techniques actuelles de caracterisation de ces proprietes sont le Dual-energy X-ray Absorptiometry (DXA) et le Quantitative Computed Tomogaphy (QCT). Ces techniques ne sont pas optimales et presentent certaines limites, ainsi l'efficacite du DXA est questionnee dans le milieu orthopedique tandis que le QCT necessite des niveaux de radiations problematiques pour en faire un outil de depistage. Les ondes guidees ultrasonores sont utilisees depuis de nombreuses annees pour detecter les fissures, la geometrie et les proprietes mecaniques de cylindres, tuyaux et autres structures dans des milieux industriels. De plus, leur utilisation est plus abordable que celle du DXA et n'engendrent pas de radiation ce qui les rendent prometteuses pour detecter les proprietes mecaniques des os. Depuis moins de dix ans, de nombreux laboratoires de recherche tentent de transposer ces techniques au monde medical, en propageant les ondes guidees ultrasonores dans les os. Le travail presente ici a pour but de demontrer le potentiel des ondes guidees ultrasonores pour determiner l'evolution des proprietes mecaniques de l'os cortical. Il commence par une introduction generale sur les ondes guidees ultrasonores et une revue de la litterature des differentes techniques relatives a l'utilisation des ondes guidees ultrasonores sur les os. L'article redige lors de ma maitrise est ensuite presente. L'objectif de cet article est d'exciter et de detecter certains modes des ondes guides presentant une sensibilite a la deterioration des proprietes mecaniques de l'os cortical. Ce travail est realise en modelisant par elements finis la propagation de ces ondes dans deux modeles osseux cylindriques. Ces deux modeles sont composes d'une couche peripherique d'os cortical et remplis soit d'os trabeculaire soit de moelle osseuse. Ces deux modeles permettent d'obtenir deux geometries, chacune propice a la propagation circonferentielle ou longitudinale des ondes guidees. Les resultats, ou trois differents modes ont pu etre identifies, sont compares avec des donnees experimentales obtenues avec des fantomes osseux et theoriques. La sensibilite de chaque mode pour les differents parametres des proprietes mecaniques est alors etudiee ce qui permet de conclure sur le potentiel de chaque mode quant a la prediction de risque de fracture ou de presence de microfractures.

Day surgery is often considered as a marker of the necessity of reorganizing the hospital to take care globally and so better meet the expectations of improvement of the management of patients. But the actual deployment of day surgery can also act as a real revelation of the stakes of conciliation between the regulations, which supervise professional practices and organization, and the functioning of hospitals. Between the regulations supervising hospitals and professional practices and the place of the recommendations, between the general legal framework of the medical activity and specific legal framework (decree of anesthesia of 1994) and the Evidence-Based Medicine, the pretext of the improvement of the patient flow in day surgery, recommended by several institutions (Sfar, ANAP, HAS), questions about the legal obligation of the passage of all the patients in the postanesthesia care unit (PACU). Seen under the angle of a legal action against a medical doctor, the study of the French jurisprudence reveals that every practitioner has to respect the recommendations and the Evidence-Based Medicine, and this in the standardized frame of the MD's activity and the respect for a very strict legal environment. The question of an obvious conciliation between all these measures arises today clearly. In the case of a potential conflict, the key of resolution, based only on legal standards (constitution, laws, decrees), is not enough for arbitrating. Applying that the only respect for the decree of anesthesia would be enough for exempting itself from any contentious risk does not satisfy more. There is a real difficulty defining the legal precise nature of the recommendations, so best practices as better organization, which are more and more frequently. Even if these recommendations originally had not their place in the hierarchy of the legal standards, they are brought in there today. There is a real brake in the deployment of the day surgery because the strict respect for the decree of 94 on the systematic passage in PACU can be paradoxical with a better quality of the care. Twenty years after the publication of the decree of anesthesia, it seems essential to ask at first if it's possible to fast-track discharge without any stay in the PACU and thus of the inevitable conciliation between all these measures. An assessment of the new monograph chapter Compounding of Radiopharmaceuticals has been conducted on the basis of the first period of implementation of Italian legislation on Good Radiopharmaceuticals Practice (NBP) in the preparation of radiopharmaceuticals, in keeping with Decree by the Italian Ministry of Health dated March 30, 2005. This approach is well grounded in the several points of similarity between the two sets of regulations. The impact on patient risk, on staff risk, and on healthcare organization risk, has been assessed. At the same time, the actual costs of coming into compliance with regulations have been estimated. A change risk analysis has been performed through the identification of healthcare-associated risks, the analysis and measurement of the likelihood of occurrence and of the potential impact in terms of patient harm and staff harm, and the determination of the healthcare organization's controlling capability. In order to evaluate the economic impact, the expenses directly related to the implementation of the activities as per ministerial decree have been estimated after calculating the overall costs unrelated to NBP implementation. The resulting costs have then been averaged over the total number of patient services delivered. NBP implementation shows an extremely positive impact on risk management for both patients receiving Nuclear Medicine services and the healthcare organization. With regard to healthcare workers, instead, the implementation of these regulations has a negative effect on the risk for greater exposure and a positive effect on the defense against litigation. The economic impact analysis of NBP implementation shows a 34% increase in the costs for a single patient service. The implementation of the ministerial decree allows for greater detectability of and control over a number of critical elements, paving the way for risk management and minimization. We, therefore, believe that the proposed tool can provide basic criteria for analysis that could be used by other organizations setting about completing the same process.

The elemental composition of surficial sediments of Venice Lagoon (Italy) in 1987, 1993, 1998 and 2003 were investigated. Zn and Cr concentrations resulted in higher than background levels, but only Cd and Hg were higher than legal quality standards (Italian Decree 2010/260 and Water Framework Directive 2000/60/EC). Contaminants with similar spatial distribution are sorted into three groups by means of correlation analysis: (i) As, Co, Cd, Cu, Fe, Pb, Zn; (ii) Ni, Cr; (iii) Hg. Interannual concentrations are compared by applying a factor analysis to the matrix of differences between subsequent samplings. A general decrease of heavy metal levels is observed from 1987 to 1993, whereas particularly high concentrations of Ni and Cr are recorded in 1998 as a consequence of intense clam fishing, subsequently mitigated by better prevention of illegal harvesting. Due to the major role played by anthropogenic sediment resuspension, bathymetric variations are also considered. Copyright © 2014 Elsevier Ltd. All rights reserved.

Education and training among Italian postgraduate medical schools in public health: a comparative analysis.

The postgraduate medical Schools in Public Health (locally known as School of Hygiene and Preventive Medicine) should ensure adequate scientific and technical knowledge and professional skills in preventive medicine, health promotion and healthcare planning as provided by Ministerial Decree 285/2005. The Italian Committee of Medical Residents in Hygiene, Preventive Medicine and Public Health of the Italian Society of Hygiene, Public Health and Preventive Medicine - S.It.I. (Consulta Nazionale dei medici in formazione specialistica S.It.I.) has always been engaged in monitoring activities on public health teaching, guaranteeing the homogeneity of educational proposals among all national Schools in Public Health. The purpose of this study is to provide a 'snapshot' of public health education and training in Italy and to identify the improvement actions needed for implementing an innovative and homogeneous public health training. A cross-sectional study was carried out over a period of three months (March to May 2013). A self-administered questionnaire was e-mailed to local Committee's delegates of all 32 postgraduate medical Schools in Public Health in Italy. The questionnaire was structured in four sections: general information, University education and training, extra-University training, interdisciplinary activities. The majority of local Committee's delegates have agreed to be enrolled in the survey. A total of 28 questionnaires were returned (88% response rate). The number of residents in each Italian School in Public Health ranged from 7 to 31. The distribution of professors in relation to residents is not similar for each University Schools. The ratio professors/residents spanning from 0.2 to 2. About teaching, only 4 University Schools offered all courses requested by Ministerial Decree 285/2005. Most of them offered at least 75% of the requested courses, but there were Schools in which the courses were less than 50%. The vast majority of schools held more than 60% of the qualifying activities considered essential according to the Decree, while 2 Schools were below 50%. All Schools required an internship of 6-12 months in local health authority offices (ASL), mainly concerning the Department of Prevention activities. In all Schools a period of stay in a Hospital Medical Direction was scheduled, while professional activities at Residential care homes were very rarely included in training programmes. Many Schools allowed residents to attend companies with biological hazard or to follow similar activities in dedicated services of ASL. Finally, in the majority of Schools, a training period in various local (Service for Water Control), regional (Departments) or national (Ministry, National Institute of Health) health facilities was contemplated and, in some cases, also in other Universities or Research Institutes. Although the Ministerial Decree indicates the essential milestones of the public health education, flexibility is seen as an important element in order to optimize resources and contextualize the adequate education of residents. In any case, at least regarding public health courses, the majority of University education and extra-University training activities should be carried out by all Schools. In order to obtain shared knowledge and skills, the Ministerial Decree should be revised taking into account flexibility and changing as intrinsic characteristics of public health profession and learners should be involved in the reform to strengthening the role of public health teaching.

L'endommagement par fatigue des materiaux est un probleme courant dans de nombreux domaines, dont celui de l'aeronautique. Afin de prevenir la rupture par fatigue des materiaux il est necessaire de determiner leur duree de vie en fatigue. Malheureusement, dues aux nombreuses heterogeneites presentes, la duree de vie en fatigue peut fortement varier entre deux pieces identiques faites dans le meme materiau ayant subi les memes traitements. Il est donc necessaire de considerer ces heterogeneites dans nos modeles afin d'avoir une meilleure estimation de la duree de vie des materiaux. Comme premiere etape vers une meilleure consideration des heterogeneites dans nos modeles, une etude en elasticite lineaire de l'influence des orientations cristallographiques sur les champs de deformations et de contraintes dans un polycristal a ete realisee a l'aide de la methode des elements finis. Des correlations ont pu etre etablies a partir des resultats obtenus, et un modele analytique en elasticite lineaire prenant en compte les distributions d'orientations cristallographiques et les effets de voisinage a pu etre developpe. Ce modele repose sur les bases des modeles d'homogeneisation classique, comme le schema auto-coherent, et reprend aussi les principes de voisinage des automates cellulaires. En prenant pour reference les resultats des analyses elements finis, le modele analytique ici developpe a montre avoir une precision deux fois plus grande que le modele auto-coherent, quel que soit le materiau etudie.

An international conference on Transposable Elements (TEs) was held 21-24 April 2012 in Saint Malo, France. Organized by the French Transposition Community (GDR Elements Génétiques Mobiles et Génomes, CNRS) and the French Society of Genetics (SFG), the conference's goal was to bring together researchers from around the world who study transposition in diverse organisms using multiple experimental approaches. The meeting drew more than 217 attendees and most contributed through poster presentations (117), invited talks and short talks selected from poster abstracts (48 in total). The talks were organized into four scientific sessions, focused on: impact of TEs on genomes, control of transposition, evolution of TEs and mechanisms of transposition. Here, we present highlights from the talks given during the platform sessions.
