Disaster Risk Transfer for Developing Countries

Financing disaster recovery often diverts resources from development, which can have long-term effects on economic growth and the poor in developing countries. Moreover, post-disaster assistance, while important for humanitarian reasons, has failed to meet the needs of developing countries in reducing their exposure to disaster risks and assuring sufficient funds to governments and individuals for financing the recovery process. The authors argue that part of disaster aid should be refocused from post-disaster to pre-disaster assistance including financial disaster risk management. Such assistance is now possible with new modeling techniques for estimating and pricing risks of natural disasters coupled with the advent of novel insurance instruments for transferring catastrophe risk to the global financial markets. The authors illustrate the potential for risk transfer in developing countries using the IIASA CATSIM model, which shows the potential impacts of disasters on economic growth in selected developing countries and the pros and cons of financial risk management to reduce those adverse impacts. The authors conclude by summarizing the advantages of investing in risk-transfer instruments (coupled with preventive measures) as an alternative to traditional post-disaster donor assistance. Donor-supported risk-transfer programs would not only leverage limited disaster aid budgets, but would also free recipient countries from depending on the vagaries of post-disaster assistance. Both the donors and the recipients stand to gain, especially since the instruments can be designed to encourage preventive measures. Precedents already exist for imaginative risk-transfer programs in highly exposed developing countries, including national insurance systems, micro-insurance schemes like weather derivatives and novel instruments (e.g., catastrophe bonds) to provide insurance cover for public sector risks.

Bogor regency consists of 40 districts, 23 are prone tonatural disasters. Klapanunggal district is listed in 10 districts declared as most vulnerable to natural disasters. Natural disasters could lead to loss of property and infrastructure damage and will affect the food security in the region. Food shortages is one example of the condition which causes food insecurity. The aim of this research is to analyze the government's food security strategy in anticipation of disaster with a case study of food insecurity in Klapanunggal district. The analysis suggested that; 1) FSVA is an appropriate program to identify food shortage areas, 2) Food Shortage Relief Program (Program Penanganan Daerah Rawan Pangan-PDRP) is the optimal efforts in reducing food shortages in the region, 3)The mismatch between FSVA indicators and Food Shortage Relief Program makes Klapanunggal district difficult in achievingfree status food-shortage. Based on the analysis, it is suggested that the implementation of Food Shortages Relief Program could be carried out based on the priority issues and implemented with integrated coordination and assistance among stakeholders. Such priority issues, integrated coordination and assistance are fully analyzed in this study.

The Volcano Disaster Assistance Program (VDAP) is a joint effort between USGS and the U.S. Agency for International Development's (USAID) Office of U.S. Foreign Disaster Assistance (OFDA). OFDA leads and coordinates disaster responses overseas for the U.S. government and is a unique stakeholder concerned with volcano disaster risk reduction as an international humanitarian assistance donor. One year after the tragic eruption of Nevado del Ruiz in 1985, OFDA began funding USGS to implement VDAP. VDAP's mission is to reduce the loss of life and property and limit the economic impact from foreign volcano crises, thereby preventing such crises from becoming disasters. VDAP fulfills this mission and complements OFDA's humanitarian assistance by providing crisis response, capacity-building, technical training, and hazard assessments to developing countries before, during, and after eruptions. During the past 30 years, VDAP has responded to more than 27 major volcanic crises, built capacity in 12+ countries, and helped counterparts save tens of thousands of lives and hundreds of millions of dollars in property. VDAP responses have evolved as host-country capabilities have grown, but the pace of work has not diminished; as a result of VDAP's work at 27 volcanoes in fiscal year 2014, more than 1.3 million people who could have been impacted by volcanic activity benefitted from VDAP assistance, 11 geological policies were modified, 188 scientists were trained, and several successful eruption forecasts were made. VDAP is developing new initiatives to help counterparts monitor volcanoes and communicate volcanic risk. These include developing the Eruption Forecasting Information System (EFIS) to learn from compiled crisis data from 30 years of VDAP responses, creating event trees to forecast eruptions at restless volcanoes, and exploring the use of unmanned aerial systems for monitoring. The use of these new methods, along with traditional VDAP assistance, has improved VDAP's ability to assist counterparts with preparing for eruptions.

This descriptive study explores students' perceptions of personal and program preparedness for disasters. Participants in this online survey included 1,348 nursing students from every state plus Guam, Puerto Rico, and theVirgin Islands. The study explored three questions: a) the level of preparedness, including learning about different types of disasters, preparing disaster plans, creating disaster kits, and participating in community disaster response efforts; b) the impact of disasters on nursing students; and c) strategies to assist nursing students during disasters. Results indicated that nursing students throughout the country are generally not well prepared for disasters. Nurse educators need to develop strategies to prepare their students for disasters. The American Red Cross provides templates for organizations, including colleges and universities, to prepare their campuses for emergencies. Faculty need to collaborate with staff and students to develop and implement plans appropriate for their programs.

To review the history and goals of the US Department of Defense's largest civilian assistance program, the Overseas Humanitarian, Disaster and Civic Aid Program and to describe the number, geographic regions, years, key words, countries, and types of projects carried out under this program since 2001. Using the program's central database, we reviewed all approved projects since 2001 and tabulated them by year, combatant command, country, and key word. We also reviewed the project descriptions of projects funded between January 1, 2006, and February 9, 2008, and examined how their activities varied by combatant command and year. Of the 5395 projects in the database, 2097 were funded. Projects took place in more than 90 countries, with Southern, Pacific, and Africa Command hosting the greatest number. The most common types of projects were school, health, disaster response, and water infrastructure construction, and disaster-response training. The "global war on terror" was the key word most frequently tagged to project descriptions. Project descriptions lacked stated goals as well as implementation and coordination strategies with potential partners, and did not report outcome or impact indicators. The geographic reach of the program is vast and projects take place in a wide variety of public sectors. Yet their security and civilian assistance value remains unclear given the lack of stated project goals, implementation strategies, or measures of effectiveness. To facilitate transparency and policy discussion, we recommend project proposals include hypotheses as to how they will enhance US security, their relevance to the public sector they address, and outcome and impact indicators that can assess their value and effectiveness.

The Central American Network for Disaster and Health Information.

This paper describes an international outreach program to support rebuilding Central America's health information infrastructure after several natural disasters in the region, including Hurricane Mitch in 1998 and two major earthquakes in 2001. The National Library of Medicine joined forces with the Pan American Health Organization/World Health Organization, the United Nations International Strategy for Disaster Reduction, and the Regional Center of Disaster Information for Latin America and the Caribbean (CRID) to strengthen libraries and information centers in Central America and improve the availability of and access to health and disaster information in the region by developing the Central American Network for Disaster and Health Information (CANDHI). Through CRID, the program created ten disaster health information centers in medical libraries and disaster-related organizations in six countries. This project served as a catalyst for the modernization of several medical libraries in Central America. The resulting CANDHI provides much needed electronic access to public health "gray literature" on disasters, as well as access to numerous health information resources. CANDHI members assist their institutions and countries in a variety of disaster preparedness activities through collecting and disseminating information.

Voice communication of information during disasters is often inadequate. In particular, simultaneous transmission by multiple callers on the same frequency can result in blocked transmissions and miscommunications. In contrast, nonvoice transmission of data requires less time than does voice communication of the same data, and may be more accurate. We conducted a pilot study to test the feasibility of a microcomputer assisted communication (MAC) network linking the disaster scene and the command hospital. The radio chosen to transmit data from the field disaster site to the command hospital was a cellular telephone connected to the microcomputer by modem. Typed communications between the microcomputer operators enabled dialogue between the disaster site and the hospitals. A computer program using commercially available software (Symphony by Lotus, Inc.) was written to allow for data entry, data transmission, and reports. Patient data, including age, sex, severity of injury, identification number, major injuries, and hospital destination were successfully transmitted from the disaster site command post to the command hospital. This pilot test demonstrated the potential applicability of MAC for facilitating transmission of patient data during a disaster.

Background Disaster medicine education is an enormous challenge, but indispensable for disaster preparedness. Aims We aimed to develop and implement a disaster medicine curriculum for medical student education that can serve as a peer-reviewed, structured educational guide and resource. Additionally, the process of designing, approving and implementing such a curriculum is presented. Methods The six-step approach to curriculum development for medical education was used as a formal process instrument. Recognized experts from professional and governmental bodies involved in disaster health care provided input using disaster-related physician training programs, scientific evidence if available, proposals for education by international disaster medicine organizations and their expertise as the basis for content development. Results The final course consisted of 14 modules composed of 2-h units. The concepts of disaster medicine, including response, medical assistance, law, command, coordination, communication, and mass casualty management, are introduced. Hospital preparedness plans and experiences from worldwide disaster assistance are reviewed. Life-saving emergency and limited individual treatment under disaster conditions are discussed. Specifics of initial management of explosive, war-related, radiological/nuclear, chemical, and biological incidents emphasizing infectious diseases and terrorist attacks are presented. An evacuation exercise is completed, and a mass casualty triage is simulated in collaboration with local disaster response agencies. Decontamination procedures are demonstrated at a nuclear power plant or the local fire department, and personal decontamination practices are exercised. Mannequin resuscitation is practiced while personal protective equipment is utilized. An interactive review of professional ethics, stress disorders, psychosocial interventions, and quality improvement efforts complete the training. Conclusions The curriculum offers medical disaster education in a reasonable time frame, interdisciplinary format, and multi-experiential course. It can serve as a template for basic medical student disaster education. Because of its comprehensive but flexible structure, it should also be helpful for other health-care professional student disaster education programs. PMID:20414376

The U.S. Agency for International Development’s (USAID) Office of U.S. Foreign Disaster Assistance (OFDA) supports several geologic-hazard related projects that help reduce the impact of geologic disasters by utilizing advances in science to monitor hazards and mitigate their effects. OFDA’s main responsibility is to rapidly respond to disasters, but OFDA also supports disaster risk reduction activities that aim to ultimately decrease the need for external responders and help to sustain development efforts by lessening the impact of potential disasters and strengthening at-risk community’s resiliency. One of OFDA’s success stories in geologic hazard risk reduction is the Volcano Disaster Assistance Program (VDAP). Following the deadly 1985 eruption of Nevado del Ruiz volcano in Colombia that killed about 25,000 people, the U.S. Geological Survey (USGS) and OFDA formed the VDAP team to provide technical assistance worldwide when potentially dangerous volcanoes show signs of unrest. VDAP also provides technical assistance for capacity-building projects at foreign observatories in order to strengthen their volcano monitoring networks and better prepare them for future activity. VDAP has deployed to 24 major crises in the past 23 years and helped to build infrastructure in 12 countries. They have helped their local counterparts save tens of thousands of lives, and hundreds of millions of dollars in property. Several factors contribute to VDAP’s success: sustained technical assistance allows VDAP to build upon previous efforts, working in the background with counterparts promotes independence, and addressing response and capacity-building needs leads to sustained development among counterpart agencies. Some of the lessons learned from VDAP will be parlayed into the newly formed OFDA-USGS Earthquake Disaster Assistance Team (EDAT), which will provide technical assistance to scientists shortly after large earthquakes occur in foreign countries so that they can “build back better” after events. An example of how OFDA is using advances in science to address the impact of earthquakes on society is the Prompt Assessment of Global Earthquakes for Response (PAGER). PAGER, which is implemented by the USGS, distributes notifications that provide an estimate of the impact of significant earthquakes shortly after they occur. The notifications include earthquake information such as location, magnitude and depth, an estimate of the number of people exposed to varying levels of shaking based on the Modified Mercalli Intensity Scale, and a description of the region’s vulnerability to earthquakes. The science-based information that PAGER provides has proven to be a valuable asset for responders who have to quickly make potentially life-saving decisions often with little data. In addition, scenarios can be run using the PAGER system that provide a visual means to communicate the potential seismic hazard to at-risk communities and decision makers so that they can make informed decisions about future development. OFDA’s disaster risk reduction projects such as VDAP, EDAT, and PAGER, help promote sustained development by lessening the impact of future geologic events.

The Central American Network for Disaster and Health Information

Purpose: This paper describes an international outreach program to support rebuilding Central America's health information infrastructure after several natural disasters in the region, including Hurricane Mitch in 1998 and two major earthquakes in 2001. Setting, Participants, and Description: The National Library of Medicine joined forces with the Pan American Health Organization/World Health Organization, the United Nations International Strategy for Disaster Reduction, and the Regional Center of Disaster Information for Latin America and the Caribbean (CRID) to strengthen libraries and information centers in Central America and improve the availability of and access to health and disaster information in the region by developing the Central American Network for Disaster and Health Information (CANDHI). Through CRID, the program created ten disaster health information centers in medical libraries and disaster-related organizations in six countries. Results/Outcome: This project served as a catalyst for the modernization of several medical libraries in Central America. The resulting CANDHI provides much needed electronic access to public health “gray literature” on disasters, as well as access to numerous health information resources. CANDHI members assist their institutions and countries in a variety of disaster preparedness activities through collecting and disseminating information. PMID:17641767

The developing world is facing an increased risk of accelerating disaster losses. A concrete risk assessment along with subsequent management program involving identification, mitigation and preparedness will assist in rehabilitation and reconstruction once the disaster has struck is critical to subvert the magnitude of the loss incurred. A developing country like India has been taken as an example to highlight the elements mentioned. Most countries like India in the developing world is facing a mounting challenge to promote economic growth and bring down poverty. In this scenario, significant climatic changes will not only impact key economic sectors but also add to the existing conundrum. Sudden onset of natural calamities pose an increasing problem to the developing countries for which risk management strategies need to be forged in order to deal with such hazards. If this is not the case, then a substantial diversion of financial resources to reconstruction in the post disaster phase severely messes up the budget planning process. This compromises economic growth in the long run. Envisaging cost effective mitigation measures to minimize environmental and socio economic toll from natural disasters is the immediate requirement. Often it has been found that an apparent lack of historical data on catastrophic events makes hazard assessment an extremely difficult process. For this it is useful to establish preliminary maps to identify high risk zones and justify the utilization of funds. Vulnerability studies assess the physical, social and economic consequences that result from the occurrence of a severe natural phenomenon. Also they take into account public awareness of risk and the consequent ability to cope with such risks. Risk analysis collates information from hazard assessment and vulnerability studies in the form of an estimation of probable future losses in the face of similar hazards. Promoting different governmental schemes to catastrophe risk absorption can be of great assistance for individuals in this context. Reconstruction and rehabilitation measures provide long term assistance for people having suffered major disaster losses. This will involve cooperation and participation of the local communities and stakeholders. In India the government is actively assisting the states in their response to catastrophes. India lacks an integrated system for disaster risk management, instead it is developing a loosely networked system. Here, the NGOs play a significant role in risk reduction programs. The National Natural Disaster Knowledge Network has been set up to promote a simultaneous interactive platform for all the stakeholders dealing with natural disasters. An Indian NGO like Disaster Mitigation Institute is closely working with the government to design means to address disaster loss. The apparent deficit in India is the dominance of the unorganized sector and there is an active focus in increasing the government's contribution by creating various national programs. Involvement of the private sector will also play a key role in addressing such losses in the future. There is an increasing emergence of various initiatives that can provide a meaningful platform to tackle the staggering losses incurred from severe natural hazard events.

To examine patterns and predictors of primary mental health care service use following 2 major Australian natural disaster events. Utilizing data from a national minimum dataset, descriptive and regression analyses were conducted to identify levels and predictors of the use of the Access to Allied Psychological Services (ATAPS) program over a 2-year period following 2 major Australian bushfire and flood/cyclone disasters. The bushfire disaster resulted in significantly greater and more enduring ATAPS service volume, while service delivery for both disasters peaked in the third quarter. Consumers affected by bushfires (IRR 1.51, 95% CI 1.20-1.89), diagnosed with depression (IRR 2.57, 95% CI 1.60-4.14), anxiety (IRR 2.06, 95% CI 1.21-3.49), or both disorders (IRR 2.15, 95% CI 1.35-3.42) utilized treatment at higher rates. The substantial demand for primary mental health care services following major natural disasters can vary in magnitude and trajectory with disaster type. Disaster-specific ATAPS services provide a promising model to cater for this demand in primary care settings. Disaster type and need-based variables as drivers of ATAPS use intensity indicate an equitable level of service use in line with the program intention. Established service usage patterns can assist with estimating capacity requirements in similar disaster circumstances.

The principles of disaster management, among others, are quick and precise, priorities, coordination and cohesion, efficient and effective manner. Help that is needed by most societies are logistical assistance, such as the assistance covers people’s everyday needs, such as food, instant noodles, fast food, blankets, mattresses etc. Logistical assistance is needed for disaster management, especially in times of disasters. The support of logistical assistance must be timely, to the right location, target, quality, quantity, and needs. Nurses are often called upon to play the role of first responder when disaster occurs. Yet the lack of accepted competencies and gaps in education make it difficult to recruit nurses prepared to respond to a disaster and provide assistance in an effective manner. Based on the International Council of Nurses (ICN) Framework of Disaster Nursing Competencies and Global Standards for the Initial Education of Professional Nurses and Midwives, a training course titled "Introduction to Disaster Nursing" was designed and implemented with 150 students. A pre-post survey design was used to assess changes in participants' self-rated disaster nursing competencies. The impact of the training course on participants' attitudes toward disaster nursing and their learning experience were also assessed. All participants passed the assessments and examination with an average score of 70%. Pre- and posttraining self-ratings of the disaster nursing competencies increased from 2.09 to 3.71 (p < .001) on a Likert scale of 1 to 5, and the effect size was large, with Cohen's d higher than 0.8. No significant difference in both examination results (60% group assignments; 40% written examination) and self-rated competencies was noted between the senior year students and graduate nurse participants by Mann-Whitney U test (p value = .90). The majority of participants indicated their willingness to participate as a helper in disaster relief and saw themselves competent to work under supervision. The ICN Framework of Disaster Nursing Competencies was instrumental to guide the training curriculum development. This introductory training course could be incorporated into undergraduate nursing education programs as well as serve as a continuing education program for graduate nurses. The training program can be used for preparing generalist nurses of their nursing competencies in disaster preparedness, response and post-disaster recovery and rehabilitation. © 2010 Sigma Theta Tau International.

Nurses' requirements for relief and casualty support in disasters: a qualitative study.

Nurses are among the most important groups engaged in casualty support, regardless of the cause, and they are one of the largest care groups involved in disasters. Consequently, these workers should gain proper support and skills to enable effective, timely, responsible and ethical emergency responses. In this study, we investigated the needs of nurses for proper casualty support in disasters, to facilitate better planning for disaster management. This was a qualitative content analysis study. Interviews were performed with 23 nurses, at educational hospitals and the Faculty of Nursing at Kerman Medical University, who had a minimum of five years working experience and assisted in an earthquake disaster. Intensity and snowball sampling were performed. Data was collected through semi-structured interviews. Interviews were transcribed and coded into main themes and subthemes. Four major themes emerged from the data; 1) psychological support, 2) appropriate clinical skills education, 3) appropriate disaster management, supervision and programming, and 4) the establishment of ready for action groups and emergency sites. The participants' comments highlighted the necessity of training nurses for special skills including emotion management, triage and crush syndrome, and to support nurses' families, provide security, and act according to predefined programs in disasters. There are a wide range of requirements for disaster aid. Proper aid worker selection, frequent and continuous administration of workshops and drills, and cooperation and alignment of different governmental and private organizations are among the suggested initiatives.
