  1. Implementation Plan for the Hawaii Geothermal Project Environmental Impact Statement (DOE Review Draft:)

    The US Department of Energy (DOE) is preparing an Environmental Impact Statement (EIS) that identifies and evaluates the environmental impacts associated with the proposed Hawaii Geothermal Project (HGP), as defined by the State of Hawaii in its 1990 proposal to Congress (DBED 1990). The location of the proposed project is shown in Figure 1.1. The EIS is being prepared pursuant to the requirements of the National Environmental Policy Act of 1969 (NEPA), as implemented by the President's Council on Environmental Quality (CEQ) regulations (40 CFR Parts 1500-1508) and the DOE NEPA Implementing Procedures (10 CFR 1021), effective May 26, 1992. The State's proposal for the four-phase HGP consists of (1) exploration and testing of the geothermal resource beneath the slopes of the active Kilauea volcano on the Island of Hawaii (Big Island), (2) demonstration of deep-water power cable technology in the Alenuihaha Channel between the Big Island and Mau, (3) verification and characterization of the geothermal resource on the Big Island, and (4) construction and operation of commercial geothermal power production facilities on the Big Island, with overland and submarine transmission of electricity from the Big Island to Oahu and possibly other islands. DOE prepared appropriate NEPA documentation for separate federal actions related to Phase 1 and 2 research projects, which have been completed. This EIS will consider Phases 3 and 4, as well as reasonable alternatives to the HGP. Such alternatives include biomass coal, solar photovoltaic, wind energy, and construction and operation of commercial geothermal power production facilities on the Island of Hawaii (for exclusive use on the Big Island). In addition, the EIs will consider the reasonable alternatives among submarine cable technologies, geothermal extraction, production, and power generating technologies; pollution control technologies; overland and submarine power transmission routes; sites reasonably suited to support

    The Title V permitting process is well underway for many industries and just beginning for others. Waste Management recently conducted the first in-depth reviews of draft Title V permits for some of its municipal solid waste (MSW) landfills. The Company`s comments on the draft permits were numerous. The draft permits contained some obvious as well as several less obvious errors. The results of these reviews identified a need to scrutinize carefully each and every draft permit issued to the Company`s landfills. The Company expects to require Title V permits for most of its 140 plus active landfills, due to the recently-enacted New Source Performance Standards for Municipal Solid Waste Landfills. Many issues of concern were encountered during the review. Had these issues not been identified, the facility would have had to live with an unnecessarily burdensome permit consisting of permit conditions that were more strict than the regulations required and lacking protections allowed by the law. This paper discusses many of the issues of concern identified during the draft permit review. Although many of the issues addressed in this paper are specific to MSW landfills, many of the issues are also applicable to Title V permits in general.

    Longer and heavier trains mean larger in-train forces and more complicated force patterns. Practical experience indicates that the development of fatigue failure of coupling systems in long heavy trains may differ from conventional understanding. The friction-type draft gears are the most widely used draft gears. The ever developing heavy haul transport environment requires further or new understanding of friction draft gear behaviour and its implications for train dynamics as well as fatigue damage of rolling stock. However, modelling of friction draft gears is a highly nonlinear question. Especially the poor predictability, repeatability and the discontinuity of friction make this task more challenging. This article reviews current techniques in dynamics modelling of friction draft gears to provide a starting point that can be used to improve existing or develop new models to achieve more accurate force amplitude and pattern predictions.

    The purpose of this ``living`` Safety Review Plan (SRP) is to describe the products and processes that will be followed to conduct a systematic review of the Accelerator Production of Tritium (APT) Facility Preliminary Safety Analysis Report (PSAR), and subsequently to prepare a draft Safety Evaluation Report (SER) on the PSAR. This plan is prepared for and will be implemented by the APT Independent Safety Review Committee (ISRC) over the period July 1, 1997 through September 30, 1998, in accordance with provisions established in DOE-STD-1104-96. A core team of DOE, INEEL, and AMPARO Corporation engineers and scientists will prepare the initial draft SER with assistance from other ISRC team members on an as needed basis. Guidelines for preparing the draft SER are presented in Section 7 of this SRP. The PSAR reviews will focus exclusively on safety. The ever-present two-part question will be: Does the subject matter have safety significance? If so, does the APT structural, system, component, and/or process engineering design ensure an acceptable margin of safety? The APT mission, efficiency, and cost are not considerations of this plan. A more detailed discussion of the review philosophy is presented in Section 5 of this SRP.

    The Department of Energy (DOE) has prepared a revision to DOE Order 5820.2A, entitled ``Radioactive Waste Management.`` DOE issued DOE Order 5820.2A in September 1988 and, as the title implies, it covered only radioactive waste forms. The proposed draft order, entitled ``Waste Management,`` addresses the management of both radioactive and nonradioactive waste forms. It also includes spent nuclear fuel, which DOE does not consider a waste. Waste forms covered include hazardous waste, high-level waste, transuranic (TRU) waste, low-level radioactive waste, uranium and thorium mill tailings, mixed waste, and sanitary waste. The Radioactive Waste Technical Support Program (TSP) of Leached Idaho Technologies Company (LITCO) is facilitating the revision of this order. The EM Regulatory Compliance Division (EM-331) has requested that TSP estimate the impacts and costs of compliance with the revised order. TSP requested Dames & Moore to aid in this assessment by comparing requirements in Draft Order 5820.2B to ones in DOE Order 5820.2A and other DOE orders and Federal regulations. The assessment started with a draft version of 5820.2B dated January 14, 1994. DOE has released three updated versions of the draft order since then (dated May 20, 1994; August 26, 1994; and January 23, 1995). Each time DOE revised the order, Dames and Moore updated the assessment work to reflect the text changes. This report reflects the January 23, 1995 version of the draft order.

In conducting its review of the draft Science Plan, the Committee on the Assessment of the NASA Science Mission Directorate 2014 Science Plan was charged to comment on the following specific areas: (1) Responsiveness to the NRC's guidance on key science issues and opportunities in recent NRC reports; (2) Attention to interdisciplinary aspects and overall scientific balance; (3) Identification and exposition of important opportunities for partnerships as well as education and public outreach; (4) Integration of technology development with the science program; (5) Clarity on how the plan aligns with SMD's strategic planning process; (6) General readability and clarity of presentation; and (7) Other relevant issues as determined by the committee.     In 1994, EPA published a report, referred to as the “Blue Book,” which lays out EPA’s current methodology for quantitatively estimating radiogenic cancer risks. A follow-on report made minor adjustments to the previous estimates and presented a partial analysis of the uncertainties in the numerical estimates. In 2006, the National Research Council of the National Academy of Sciences released a report on the health risks from exposure to low levels of ionizing radiation. Cosponsored by the EPA and several other Federal agencies, Health Risks from Exposure to Low Levels of Ionizing Radiation BEIR VII Phase 2 (BEIR VII) primarily addresses cancer and genetic risks from low doses of low-LET radiation. In the draft White Paper: Modifying EPA Radiation Risk Models Based on BEIR VII (White Paper), ORIA proposed changes in EPA’s methodology for estimating radiogenic cancers, based on the contents of BEIR VII and some ancillary information. For the most part, it proposed to adopt the models and methodology recommended in BEIR VII; however, certain modifications and expansions are considered to be desirable or necessary for EPA’s purposes.     The EPA Draft Document on EMF and Cancers grew out of an earlier effort by EPA to track biological effects literature relative to radio-frequency (RF) exposure. Scope of the document was broadened to include extremely low frequency electric and magnetic fields prior to an announcement in 1986 that EPA would formally review the whole area of non-ionizing radiation. An extensive survey of the relevant bioeffects and epidemiologic literature was carried out, and writing on the document began in earnest on the document sometime in 1989. In its draft form, the document reviewed the literature on mechanisms of Interaction between electromagnetic fields and biological tissue, EMF epidemiologic studies, supporting evidence for carcinogenicity and research needs. In the early summer of 1990, a draft of the document was reviewed by some 22 individuals within the EPA and other government agencies. It was also sent out for external review to an additional 9 qualified scientists who had worked in the area and were familiar with EMF-related literature in epidemiology, biology, and physics. Many of the comments sent to EPA from this first review, prior to release of the draft for public comment, were strikingly similar to those resulting from the second (public) review process. Thus, it appears that much of the controversy that was later associated with the document could have been avoided had the authors been diligent in following the recommendations of the initial set of reviewers.

    Sometimes the opinion regarding the cause of death in "John Doe or Jane Doe" i.e. on unknown dead bodies is a test of ability of the forensic expert and on many occasions it yields little or no results. Here the identification of the body as such poses problems; rest aside the opinion regarding the cause/ manner of death. The present 5yr study was undertaken in the Department of Forensic Medicine & Toxicology, Government Medical College & Hospital, Chandigarh to find the patterns of cause of death in unknown dead bodies, as very little literature is available with regard to John Doe or Jane Doe cases as a group, in India. Unidentified bodies comprised 4 % of the total 3165 cases brought for post-mortem examination to the department. Maximum cases belonged to the age group 41 - 50 years, 30 %. Majority of the opinions regarding the cause of death were given as "no definite opinion" (31%), followed by "cranio-cerebral damage" (30 %) and coronary insufficiency/ Cardiac disease/ aortic aneurysm rupture, (8.9%). Following measures should be undertaken to increase the chances of getting these unknown bodies identified and thereby increasing the chances of arriving at a definite cause of death: drafting of additional legislation for the management of unidentified dead bodies along with streamlining of work on the part of police, use of active investigation and modern investigative techniques, fixing the accountability of the police. Internet based sites of the police like ZIPNET (Zonal Integrated Police Networking) in Northern India, should also be used.

    For many years, the U.S. Department of Energy (DOE) has supported and sponsored various types of water research relating to the oil and gas industry through its Office of Fossil Energy and its National Energy Technology Laboratory (NETL). In early 2005, the Petroleum Environmental Research Forum (PERF) submitted a proposal to DOE for funding an upcoming PERF meeting that would feature water research in the petroleum industry. PERF is a nonprofit organization created in 1986 to provide a stimulus to and a forum for the collection, exchange, and analysis of research information related to the development of technology concerning the petroleum industry, and a mechanism for establishing joint research projects in that field. Additional information on PERF can be accessed at http://www.perf.org. DOE agreed to provide funding to hold a review of its water research program in conjunction with the fall 2005 PERF meeting. Argonne National Laboratory (Argonne) was asked to coordinate and host the meeting, which was referred to as the DOE/PERF Water Program Review. The program review was held on November 1-4, 2005, in Annapolis, Maryland, at the Historic Inns of Annapolis. The purpose of the program review was to provide a forum for sharing information, reviewing current programs (especially recent unpublished research), and reviewing industry and regulatory needs regarding water use and reuse issues. PERF and DOE/NETL can use this information to plan for future water-related research projects. The water program review provided a unique opportunity in several ways. First, DOE was able to have all of the contractors currently receiving DOE funds for water research present in one room at the same time. Each contractor described his or her research and was able to learn about the research being conducted by the other researchers.     Multitarget stool DNA (mt-sDNA) testing was approved for average risk colorectal cancer (CRC) screening by the United States Food and Drug Administration and thereafter reimbursed for use by the Medicare program (2014). The United States Preventive Services Task Force (USPSTF) October 2015 draft recommendation for CRC screening included mt-sDNA as an “alternative” screening test that “may be useful in select clinical circumstances”, despite its very high sensitivity for early stage CRC. The evidence supporting mt-sDNA for routine screening use is robust. The clinical efficacy of mt-sDNA as measured by sensitivity, specificity, life-years gained (LYG), and CRC deaths averted is similar to or exceeds that of the other more specifically recommended screening options included in the draft document, especially those requiring annual testing adherence. In a population with primarily irregular screening participation, tests with the highest point sensitivity and reasonable specificity are more likely to favorably impact CRC related morbidity and mortality than those depending on annual adherence. This paper reviews the evidence supporting mt-sDNA for routine screening and demonstrates, using USPSTF’s modeling data, that mt-sDNA at three-year intervals provides significant clinical net benefits and fewer complications per LYG than annual fecal immunochemical testing, high sensitivity guaiac based fecal occult blood testing and 10-year colonoscopy screening.   13. Does sunlight prevent cancer? A systematic review.

    Accumulating evidence for beneficial effects of sunlight on several types of cancer with a high mortality rate makes it necessary to reconsider the health recommendations on sun exposure, which are now mainly based on the increased risks for skin cancer. We reviewed all published studies concerning sun exposure and cancer, excluding skin cancer. All selected studies on prostate (3 ecologic, 3 case-control and 2 cohort), breast (4 ecologic, 1 case-control and 2 cohort) and ovary cancer (2 ecologic and 1 case-control) showed a significantly inverse correlation between sunlight and mortality or incidence. Two ecologic, 1 case-control and 2 prospective studies showed an inverse relation between sunlight and colon cancer mortality; 1 case-control study found no such association. Ecologic studies on non-Hodgkin lymphoma (NHL) mortality and sunlight gave conflicting results: early studies showing mostly positive and later studies showing mostly negative correlations. Three case-control studies and 1 cohort study found a significant inverse association between the incidence of NHL and sunlight. The question of how to apply these findings to (public) health recommendations is discussed.

Pulling the Neanderthal sequences out of a sea of contaminating environmental DNA impregnating the bones and at the same time avoiding the problems of contamination with modern human DNA is in itself a remarkable accomplishment. However, the crucial question in the long run is, what can we learn from such genomic data about hominid evolution?

    The European Federation of Statisticians in the Pharmaceutical Industry (EFSPI) engages more than 2000 statisticians through its ten national organizations. Amongst other things, EFSPI is involved in reviewing regulatory guidelines under development, including the draft FDA guideline on non-inferiority clinical trials. This review resulted in several critical comments relating to as follows: (i) the lack of one single standard for proving efficacy of new drugs implied by the guideline; (ii) the problems with the suggested 'fraction of effect to be preserved'; (iii) the formulation of the primary hypothesis in a non-inferiority trial aiming at indirectly demonstrating a new drug is superior to placebo; and (iv) the preference in the guideline for the fixed-margin method over the synthesis method in the analysis. The presumed implications of this guideline, if implemented as is, are (i) increased confusion of how efficacy could be demonstrated when placebo control is not available, (ii) more complicated communication between pharmaceutical industry and FDA because of the apparent disagreements on fundamental statistical matters, and (iii) illogical consequences in the approval process because of which order drugs are approved rather than how they fulfill the regulatory requirements. We believe that the area is not yet ready for such a prescriptive regulatory guidance and that further research and experience are required until the methodology can be finally agreed. A strategy needs to be developed by regulatory agencies together with drug industry and academia for a long term solution for this topic.

The EEG was established in 1978 with funds provided by the U.S. Department of Energy (DOE) to the State of New Mexico. Public Law 100-456, the National Defense Authorization Act, Fiscal Year 1989, Section 1433, assigned EEG to the New Mexico Institute of Mining and Technology and continued the original contract DE-AC04-79AL10752 through DOE contract DE-AC04-89AL58309. The National Defense Authorization Act for Fiscal Year 1994, Public Law 103-160, continues the authorization. EEG performs independent technical analyses of the suitability of the proposed site; the design of the repository, its planned operation, and its long-term integrity; suitability and safety of the transportation systems; suitability of the Waste Acceptance Criteria and the generator sites` compliance with them; and related subjects. These analyses include assessments of reports issued by the DOE and its contractors, other federal agencies and organizations, as they relate to the potential health, safety and environmental impacts from WIPP. Another important function of EEG is the independent environmental monitoring of background radioactivity in air, water, and soil, both on-site and off-site.

    Hanford Environmental Dose Reconstruction (HEDR) Project staff are developing mathematical models to be used to estimate the radiation dose that individuals may have received as a result of emissions since 1944 from the US Department of Energy`s (DOE) Hanford Site near Richland, Washington. An uncertainty and sensitivity analyses plan is essential to understand and interpret the predictions from these mathematical models. This is especially true in the case of the HEDR models where the values of many parameters are unknown. This plan gives a thorough documentation of the uncertainty and hierarchical sensitivity analysis methods recommended for use on all HEDR mathematical models. The documentation includes both technical definitions and examples. In addition, an extensive demonstration of the uncertainty and sensitivity analysis process is provided using actual results from the Hanford Environmental Dose Reconstruction Integrated Codes (HEDRIC). This demonstration shows how the approaches used in the recommended plan can be adapted for all dose predictions in the HEDR Project.

    Advanced human-system interface (HSI) technologies are being developed in the commercial nuclear power industry. These HSIs may have significant implications for plant safety in that they will affect the ways in which the operator interacts with and supervises an increasingly complex system. The U.S. Nuclear Regulatory Commission (NRC) reviews the HSI aspects of nuclear plants to ensure that operator performance and reliability are supported. The NRC is developing guidance to support its review of these advanced designs. The guidance consists of an evaluation methodology and an extensive set of human factors guidelines which are used in one aspect of the evaluation. The paper describes the guidance development of the evaluation methodology and the guidelines.

    The specific protection goal, primary assessment endpoints, acceptable effect thresholds, and experimental design proposed in the European Food Safety Authority (EFSA) update of the bee guidance document are subjected to critical review. It is concluded that the negligible effect criteria were established without sufficient regulatory definition and without convincing scientific argumentation. For the assessment endpoints, effects on hive strength lack temporal definition and the reduction to numbers of bees is inappropriate to evaluate effects. Restricting mortality assessments to homing failure is not theoretically justified and specific criteria were incorrectly derived. The combination of acute effect estimates with models for chronic stressors is biased risk assessment and a temporal basis for the acceptability of effects is missing. Effects on overwintering success cannot be experimentally assessed using the proposed criteria. The experimental methodology proposed is inappropriate and the logistical consequences, in particular those related to replication and land use are such that field studies are no longer a feasible option for the risk assessment. It may be necessary to explore new lines of thought for the set-up of field studies and to clearly separate experimentation from monitoring.     Interest in the well-being of physicians has increased because of their contributions to the healthcare system quality. There is growing recognition that physicians are exposed to workplace factors that increase the risk of work stress. Long-term exposure to high work stress can result in burnout. Reports from around the world suggest that about one-third to one-half of physicians experience burnout. Understanding the outcomes associated with burnout is critical to understanding its affects on the healthcare system. Productivity outcomes are among those that could have the most immediate effects on the healthcare system. This systematic literature review is one of the first to explore the evidence for the types of physician productivity outcomes associated with physician burnout. It answers the question, "How does burnout affect physician productivity?" A systematic search was performed of: Medline Current, Medline in process, PsycInfo, Embase and Web of Science. The search period covered 2002 to 2012. The searches identified articles about practicing physicians working in civilian settings. Articles that primarily looked only at residents or medical students were excluded. Productivity was captured by hours worked, patients seen, sick leave, leaving the profession, retirement, workload and presenteeism. Studies also were excluded if: (1) the study sample was not comprised of at least 50% physicians, (2) the study did not examine the relationship between burnout and productivity or (3) a validated measure of burnout was not used. The search identified 870 unique citations; 5 met the inclusion/exclusion criteria. This review indicates that globally there is recognition of the potential impact of physician burnout on productivity. Productivity was examined using: number of sick leave days, work ability, intent to either continue practicing or change jobs. The majority of the studies indicate there is a negative relationship between burnout and productivity. However

    The Defense Nuclear Facilities Safety Board (DNFSB) Recommendation 94-2 includes a recommendation that the Department of Energy (DOE) conduct a comprehensive, complex-wide review of the low-level waste issue to establish the dimensions of the low-level waste problem and to identify necessary corrective actions to address the safe disposition of past, present, and future volumes. DOE`s Implementation Plan calls for the conduct of a complex-wide review of low-level radioactive waste treatment, storage, and disposal sites to identify environmental, safety, and health vulnerabilities. The complex-wide review focuses on low-level waste disposal facilities through a site evaluation survey, reviews of existing documentation, and onsite observations. Low-level waste treatment and storage facilities will be assessed for their ability to meet waste acceptance criteria for disposal. Results from the complex-wide review will be used to form the basis for an integrated and planned set of actions to correct the identified vulnerabilities and to prompt development of new requirements for managing low-level waste.

    This executive summary addresses the activities associated with the National Transuranic (TRU) Program managed by the Carlsbad Area Office (CAO). The CAO programmatically reports to the Assistant Secretary for Environmental Management and receives administrative support through the Albuquerque Operations Office. The mission of the Carlsbad Area Office (CAO) is to protect human health and the environment by opening and operating the Waste Isolation Pilot Plant for site disposal of TRU waste and by establishing an effective system for management of TRU waste from generation to disposal. It includes personnel assigned to the CAO, the Waste Isolation Pilot Plant (WIPP) site operations, and other activities associated with the National TRU Program. The CAO develops and directs implementation of the program, while the DOE Headquarters establishes policy and guidelines. The CAO assesses compliance with the program guidance, as well as the commonality of activities and assumptions among all the sites. Since the development of the February 28, 1997, database used to develop this Discussion Draft, the opening of the WIPP facility for receipt of Contact Handled waste has been delayed from November 1997 to May 1998. This slippage is significant enough to require a change in the milestones and volumes included in the documents to be reviewed by our stakeholders. Changes have been incorporated into this Discussion Draft and its supporting Project Baseline Summaries (PBSs).

    This report describes a peer review of the draft Handbook for Human Reliability Analysis with Emphasis on Nuclear Power Plant Applications, NUREG/CR-1278. The purpose of the study was to determine to what extent peers agree with the human behavior models and estimates of human error probabilities (HEPs) contained in the Handbook. Twenty-nine human factors experts participated in the study. Twenty of the participants were Americans; nine were from other countries. The peers performed human reliability analyses of a variety of human performance scenarios describing operator activities in nuclear power plant settings. They also answered questionnaires pertaining to the contents and application of the Handbook. An analysis of peer solutions to the human reliability analysis problems and peer responses to the questionnaire was performed. Recommendations regarding the format and contents of the Handbook were developed from the study findings.

    This volume of the draft Mercury Study Report to Congress summarizes the available information on human health effects and animal data for hazard identification and dose-response assessment for three forms of mercury: elemental mercury, mercury chloride (inorganic mercury), and methylmercury (organic mercury). Effects are summarized by endpoint. The risk assessment evaluates carcinogenicity, mutagenicity, developmental toxicity and general systemic toxicity of these chemical species of mercury. Toxicokinetics (absorption, distribution, metabolism and excretion) are described for each of the three mercury species. PBPK models are described, but not applied in risk assessment. Reference doses are calculated for inorganic and methylmercury; a reference concentration for inhaled elemental mercury is provided. A quantitiative analysis of factors contributing to variability and uncertainty in the methylmercury RfD is provided in an appendix. Interations and sensitive populations are described.

  10. A review of DOE HEPA filter component test activities

    All HEPA filters purchased for installation in DOE nuclear facilities are required to be tested at a Filter Test Facility (FTF) prior to installation. The number of HEPA filters purchased by DOE has been reduced so much that the Hanford FTF was closed. From Fiscal Year (FY) 1992 to 1994, funding was not provided to the FTF Technical Support Group (TSG) at the Los Alamos National Laboratory. As a consequence, Round Robin Tests (RRTs), performed twice each year by the FTFs to assess constituency of test results among the FTFs, were not performed in FY 1992 and FY 1993. The Annual Reports of FTF test activities were not prepared for FY 1992 - 1995. Technical support provided to the FTFs was minimal. There is talk of closing a second FTF, and ongoing discussions as to whether DOE will continue to fund operation of the FTFs. In FY 1994, DOE Defense Programs commenced funding the TSG. RRT data for FY 1994 and 1995 have been entered into the database; the FY 1994 RRT report has been issued; and the FY 1995 RRT report is in progress. Data from semiannual reports have been retrieved and entered into the database. Standards related to HEPA filter test and procurement activities are now scheduled for issuance by FY 1996. Continuation of these activities depends on whether DOE will continue to support the HEPA filter test program. The history and activities of the FTFs and the TSG at Los Alamos have been reported at previous Air Cleaning Conferences. Data from the FY 1991 Annual Report of FTF activities was presented at the 1992 Air Cleaning Conference. Preparation of the Annual Reports was temporarily suspended in 1992. However, all of the FTF Semiannual report data have been retrieved and entered into the data base. This paper focuses primarily on the results of HEPA filter tests conducted by FTFs during FY 1992 - FY 1995, and the possible effects of the DOE program uncertainties on the quality of HEPA filters for installation at the DOE sites. 15 refs., 13 tabs.
