    Emergency medicine (EM) has an important role in public health, but the ideal approach for teaching public health to EM residents is unclear. As part of the national regional public health–medicine education centers-graduate medical education (RPHMEC-GM) initiative from the CDC and the American Association of Medical Colleges, three EM programs received funding to create public health curricula for EM residents. Curricula approaches varied by residency. One program used a modular, integrative approach to combine public health and EM clinical topics during usual residency didactics, one partnered with local public health organizations to provide real-world experiences for residents, and one drew on existing national as well as departmental resources to seamlessly integrate more public health–oriented educational activities within the existing residency curriculum. The modular and integrative approaches appeared to have a positive impact on resident attitudes toward public health, and a majority of EM residents at that program believed public health training is important. Reliance on pre-existing community partnerships facilitated development of public health rotations for residents. External funding for these efforts was critical to their success, given the time and financial restraints on residency programs. The optimal approach for public health education for EM residents has not been defined. PMID:21961671

    The application of different EMS current thresholds on muscle activates not only the muscle but also peripheral sensory axons that send proprioceptive and pain signals to the cerebral cortex. A 32-channel time-domain fNIRS instrument was employed to map regional cortical activities under varied EMS current intensities applied on the right wrist extensor muscle. Eight healthy volunteers underwent four EMS at different current thresholds based on their individual maximal tolerated intensity (MTI), i.e., 10 % < 50 % < 100 % < over 100 % MTI. Time courses of the absolute oxygenated and deoxygenated hemoglobin concentrations primarily over the bilateral sensorimotor cortical (SMC) regions were extrapolated, and cortical activation maps were determined by general linear model using the NIRS-SPM software. The stimulation-induced wrist extension paradigm significantly increased activation of the contralateral SMC region according to the EMS intensities, while the ipsilateral SMC region showed no significant changes.   13. Designing Better Scaffolding in Teaching Complex Systems with Graphical Simulations

    Complex systems are an important topic in science education today, but they are usually difficult for secondary-level students to learn. Although graphic simulations have many advantages in teaching complex systems, scaffolding is a critical factor for effective learning. This dissertation study was conducted around two complementary research questions on scaffolding: (1) How can we chunk and sequence learning activities in teaching complex systems? (2) How can we help students make connections among system levels across learning activities (level bridging)? With a sample of 123 seventh-graders, this study employed a 3x2 experimental design that factored sequencing methods (independent variable 1; three levels) with level-bridging scaffolding (independent variable 2; two levels) and compared the effectiveness of each combination. The study measured two dependent variables: (1) knowledge integration (i.e., integrating and connecting content-specific normative concepts and providing coherent scientific explanations); (2) understanding of the deep causal structure (i.e., being able to grasp and transfer the causal knowledge of a complex system). The study used a computer-based simulation environment as the research platform to teach the ideal gas law as a system. The ideal gas law is an emergent chemical system that has three levels: (1) experiential macro level (EM) (e.g., an aerosol can explodes when it is thrown into the fire); (2) abstract macro level (AM) (i.e., the relationships among temperature, pressure and volume); (3) micro level (Mi) (i.e., molecular activity). The sequencing methods of these levels were manipulated by changing the order in which they were delivered with three possibilities: (1) EM-AM-Mi; (2) Mi-AM-EM; (3) AM-Mi-EM. The level-bridging scaffolding variable was manipulated on two aspects: (1) inserting inter-level questions among learning activities; (2) two simulations dynamically linked in the final learning activity. Addressing the first research question, the Experiential macro-Abstract macro-Micro (EM-AM-Mi) sequencing method, following the "concrete to abstract" principle, produced better knowledge integration while the Micro-Abstract macro-Experiential macro (Mi-AM-EM) sequencing method, congruent with the causal direction of the emergent system, produced better understanding of the deep causal structure only when level-bridging scaffolding was provided. The Abstract macro-Micro-Experiential macro (AM-Mi-EM) sequencing method produced worse performance in general, because it did not follow the "concrete to abstract" principle, nor did it align with the causal structure of the emergent system. As to the second research question, the results showed that level-bridging scaffolding was important for both knowledge integration and understanding of the causal structure in learning the ideal gas law system.

    Yellowstone National Park hosts over 10,000 thermal features (e.g. geysers, fumaroles, mud pots, and hot springs), yet little is known about the circulation depth of meteoric water feeding these features, nor the lithological and structural bounds on the pathways that guide deep, hot fluids to the surface. Previous near-surface geophysical studies have been effective in imaging shallow hydrothermal pathways in some areas of the park, but these methods are difficult to conduct over the large areas needed to characterize entire hydrothermal systems. Transient electromagnetic (TEM) soundings and 2D direct current (DC) resistivity profiles show that hydrothermal fluids at active sites have a higher electrical conductivity than the surrounding hydrothermally inactive areas. For that reason, airborne TEM is an effective method to characterize large areas and identify hydrothermally active and inactive zones using electrical conductivity. Aeromagnetic data have been useful in mapping faults that localize hot springs, making the integration of aeromagnetic and EM data effective for structurally characterizing fluid pathways. Here we present the preliminary results from an airborne transient electromagnetic (TEM) and magnetic survey acquired jointly by the U.S. Geological Survey (USGS) and the University of Wyoming (UW) in November 2016. We integrate the EM and magnetic data for the purpose of edge detection of rhyolite flow boundaries as well as source depth of hydrothermal features. The maximum horizontal gradient technique applied on magnetic data is a useful tool that used to estimate source depth as well as indicate faults and fractures. The integration of EM with magnetics allows us to distinguish hydrothermally altered fault systems that guide fluids in the subsurface. We have used preliminary 2D inversions of EM from Aarhus Workbench to delineate rhyolite flow edges in the upper 300-600 meters and cross-checked those boundaries with the aeromagnetic map.

    The marine controlled source electromagnetic (MCSEM) method has become widely used in offshore oil and gas exploration. Interpretation of MCSEM data is still a very challenging problem, especially if one would like to take into account the realistic 3D structure of the subsurface. The inversion of MCSEM data is complicated by the fact that the EM response of a hydrocarbon-bearing reservoir is very weak in comparison with the background EM fields generated by an electric dipole transmitter in complex geoelectrical structures formed by a conductive sea-water layer and the terranes beneath it. In this paper, we present a review of the recent developments in the area of large-scale 3D EM forward modeling and inversion. Our approach is based on using a new integral form of Maxwell’s equations allowing for an inhomogeneous background conductivity, which results in a numerically effective integral representation for 3D EM field. This representation provides an efficient tool for the solution of 3D EM inverse problems. To obtain a robust inverse model of the conductivity distribution, we apply regularization based on a focusing stabilizing functional which allows for the recovery of models with both smooth and sharp geoelectrical boundaries. The method is implemented in a fully parallel computer code, which makes it possible to run large-scale 3D inversions on grids with millions of inversion cells. This new technique can be effectively used for active EM detection and monitoring of the subsurface targets.

    The assessment of the regional network for ST-segment elevation acute myocardial infarction (STEMI) is fundamental for quality assurance. Since 2011 all Italian Health Authorities, in addition to hospital discharge records (HDR), must provide a standardized information flow (ERD) about emergency department (ED) and emergency medical system (EMS) activities. The aim of this study was to evaluate whether data integration of ERD with HDR may allow the development of appropriate quality indicators. Patients admitted to coronary care units (CCU) for STEMI between January 1 to December 31, 2013, were identified from the regional HDR database. All data were linked to those of the regional ERD database. Four quality indicators were defined: 1) rates of EMS activation, 2) rates of EMS direct transfer to the catheterization laboratory (Cath-lab), 3) transfer rates from a Spoke to a Hub hospital with angioplasty facilities, and 4) median time spent in ED. In 2013, 2793 patients with STEMI were admitted to the CCU. Of these, 1684 patients (60%) activated EMS and were transported to Spoke or Hub hospitals; 955 (57%) entered directly in CCU/Cath-lab; 677 were transferred directly to a Hub hospital ED without being admitted to a Spoke hospital. The median ED time in Hub hospital was 47 min (IQR 24-136) and in Spoke hospital 53 min (IQR 30-131). The integration among administrative data banks (i.e., HDR with ERD) allowed the assessment of the regional STEMI network and the identification of potentially useful quality indicators. Their easy availability should enable comparisons with local, national and international standards, and may favor quality improvement.

    The Space Station Furnace Facility (SSFF) is a facility located in the International Space Station United States Laboratory (ISS US Lab) for materials research in the microgravity environment. The SSFF will accommodate basic research, commercial applications, and studies of phenomena of metals and alloys, electronic and photonic materials, and glasses and ceramics. To support this broad base of research requirements, the SSFF will operate, regulate, and support a variety of Experiment Modules (EMs). To meet station requirements concerning the microgravity level needed for experiments, station is providing an active vibration isolation system, and SSFF provides the interface. SSFF physically consists of a Core Rack and two instrument racks (IRs) that occupy three adjacent ISS US Lab rack locations within the International Space Station (ISS). All SSFF racks are modified International Standard Payload Racks (ISPR). SSFF racks will have a 50% larger pass through area on the lower sides than ISPRs to accommodate the many rack to rack interconnections. The Instrument Racks are further modified with lowered floors and an additional removable panel (15" x 22") on top of the rack for access if needed. The Core Rack shall contain all centralized Core subsystems and ISS subsystem equipment. The two Instrument Racks shall contain the distributed Core subsystem equipment, ISS subsystem equipment, and the EMs. The Core System, which includes the Core Rack, the IR structures, and subsystem components located in the IRs serves as the central control and management for the IRs and the EMs. The Core System receives the resources provided by the International Space Station (ISS) and modifies, allocates, and distributes these resources to meet the operational requirements of the furnace. The Core System is able to support a total of four EMs and can control, support, and activate/deactivate the operations of two EMs, simultaneously. The IRs can be configured to house two small EMs or one tall vertical EM, and serve as the interface between the Core and the respective EM. The Core Rack and an adjacent Instrument Rack (containing one or more furnaces) will be delivered to the ISS in one launch. This is Integrated Configuration One (ICI). The Core Rack and IRI will be passive during transport in the Mini Pressurized Logistics Module (MPLM): Any subsequent EMs to operate within IRI are installed on-orbit. The second IR (containing one or more furnaces) is delivered to ISS on a subsequent launch which will establish Integrated Configuration Two (IC2). Additional integrated configurations will be established with the replacement of EMs or Instrument Racks.

    The use of 1/f noise measurements is explored for the purpose of finding faster techniques for electromigration (EM) characterization in advanced microelectronic interconnects, which also enable a better understanding of its underlying physical mechanisms. Three different applications of 1/f noise for EM characterization are explored. First, whether 1/f noise measurements during EM stress can serve as an early indicator of EM damage. Second, whether the current dependence of the noise power spectral density (PSD) can be used for a qualitative comparison of the defect concentration of different interconnects and consequently also their EM lifetime t50. Third, whether the activation energiesmore » obtained from the temperature dependence of the 1/f noise PSD correspond to the activation energies found by means of classic EM tests. In this paper, the 1/f noise technique has been used to assess and compare the EM properties of various advanced integration schemes and different materials, as they are being explored by the industry to enable advanced interconnect scaling. More concrete, different types of copper interconnects and one type of tungsten interconnect are compared. The 1/f noise measurements confirm the excellent electromigration properties of tungsten and demonstrate a dependence of the EM failure mechanism on copper grain size and distribution, where grain boundary diffusion is found to be a dominant failure mechanism.« less

    The Energy and Environmental Research Center (EERC), in conjunction with the Waste Policy Institute (WPI), will identify and integrate new technologies to meet site-specific environmental management (EM) requirements at contaminated sites appropriate to U.S. Department of Energy (DOE) interests.     The Barite Hills/Nevada Gold Fields mines are in Late Proterozoic and early Paleozoic rocks of the gold and iron sulfides rich Carolina slate belt. The mines were active from 1989 to1995. EPA and USGS site investigations in 2003 resulted in the declaration of the waste pit areas as a superfund site. The USGS and private consulting firms have evaluated subsurface water flow paths, faults & other groundwater-related features at this superfund site utilizing 2-D conductivity & 3-D electromagnetic (EM) surveys. The USGS employed conductivity to generate instantaneous 2-D profiles to evaluate shallow groundwater patterns. Porous regolith sediments, contaminated water & mine debris have high conductivity whereas bedrock is identified by its characteristic low conductivity readings. Consulting contractors integrated EM technology, magnetic & shallow well data to generate 3-D images of groundwater flow paths at given depths across the superfund site. In so doing several previously undetected faults were identified. Lighting strike data was integrated with the previously evaluated electrical and EM data to determine whether this form of natural-sourced EM data could complement and supplement the more traditional geophysical data described above. Several lightning attributes derived from 3-D lightning volumes were found to correlate to various features identified in the previous geophysical studies. Specifically, the attributes Apparent Resistivity, Apparent Permittivity, Peak Current & Tidal Gravity provided the deepest structural geological framework & provided insights into rock properties & earth tides. Most significantly, Peak Current showed remarkable coincidence with the preferred groundwater flow map identified by one of the contractors utilizing EM technology. This study demonstrates the utility of robust integrated EM technology applications for projects focused on hydrology, geohazards to dams, levees, and structures, as well as mineral and hydrocarbon exploration.

    Reducing delays in time to treatment is a key goal of ST-elevation myocardial infarction (STEMI) emergency care. Emergency medical services (EMS) are a critical component of the STEMI chain of survival. We sought to assess the impact of the careful integration of EMS as a strategy for improving systemic treatment times for STEMI. We conducted a study of all 747 nontransfer STEMI patients who underwent primary percutaneous coronary intervention (PCI) in Dallas County, Texas from October 1, 2010 through December 31, 2011. EMS leaders from 24 agencies and 15 major PCI receiving hospitals collected and shared common, de-identified patient data. We used 15 months of data to develop a generalized linear regression to assess the impact of EMS on two treatment metrics-hospital door to balloon (D2B) time, and symptom onset to arterial reperfusion (SOAR) time, a new metric we developed to assess total treatment times. We found statistically significant reductions in median D2B (11.1-min reduction) and SOAR (63.5-min reduction) treatment times when EMS transported patients to the receiving facility, compared to self-transport. In addition, when trained EMS paramedics field-activated the cardiac catheterization laboratory using predefined specified protocols, D2B times were reduced by 38% (43 min) after controlling for confounding variables, and field activation was associated with a 21.9% reduction (73 min) in the mean SOAR time (both with p < 0.001). Active EMS engagement in STEMI treatment was associated with significantly lower D2B and total coronary reperfusion times. Copyright © 2014 Elsevier Inc. All rights reserved.

    The Office of Environmental Restoration and Waste Management (EM) was established by the Department of Energy (DOE) to direct and coordinate waste management and site remediation programs/activities throughout the DOE complex. In order to successfully achieve the goal of properly managing waste and the cleanup of the DOE sites, the EM was divided into five organizations: the Office of Planning and Resource Management (EM-10); the Office of Environmental Quality Assurance and Resource Management (EM-20); the Office of Waste Operations (EM-30); the Office of Environmental Restoration (EM-40); and the Office of Technology and Development (EM-50). The mission of the Office ofmore » Technology Development (OTD) is to develop treatment technologies for DOE`s operational and environmental restoration wastes where current treatment technologies are inadequate or not available. The Mixed Waste Integrated Program (MWIP) was created by OTD to assist in the development of treatment technologies for the DOE mixed low-level wastes (MLLW). The MWIP has established five Technical Support Groups (TSGs) whose purpose is to identify, evaluate, and develop treatment technologies within five general technical areas representing waste treatment functions from initial waste handling through generation of final waste forms.     This study presents the design and development of a mobile wireless platform to be used for monitoring and analysis of seismic events and related electromagnetic (EM) signals, employing Personal Digital Assistants (PDAs). A prototype custom-developed application was deployed on a 3G enabled PDA that could connect to the FTP server of the Institute of Geodynamics of the National Observatory of Athens and receive and display EM signals at 4 receiver frequencies (3 KHz (E-W, N-S), 10 KHz (E-W, N-S), 41 MHz and 46 MHz). Signals may originate from any one of the 16 field-stations located around the Greek territory. Employing continuous recordings of EM signals gathered from January 2003 till December 2007, a Support Vector Machines (SVM)-based classification system was designed to distinguish EM precursor signals within noisy background. EM-signals corresponding to recordings preceding major seismic events (Ms≥5R) were segmented, by an experienced scientist, and five features (mean, variance, skewness, kurtosis, and a wavelet based feature), derived from the EM-signals were calculated. These features were used to train the SVM-based classification scheme. The performance of the system was evaluated by the exhaustive search and leave-one-out methods giving 87.2% overall classification accuracy, in correctly identifying EM precursor signals within noisy background employing all calculated features. Due to the insufficient processing power of the PDAs, this task was performed on a typical desktop computer. This optimal trained context of the SVM classifier was then integrated in the PDA based application rendering the platform capable to discriminate between EM precursor signals and noise. System's efficiency was evaluated by an expert who reviewed 1/ multiple EM-signals, up to 18 days prior to corresponding past seismic events, and 2/ the possible EM-activity of a specific region employing the trained SVM classifier. Additionally, the proposed architecture can form a base platform for a future integrated system that will incorporate services such as notifications for field station power failures, disruption of data flow, occurring SEs, and even other types of measurement and analysis processes such as the integration of a special analysis algorithm based on the ratio of short term to long term signal average.

    The Department of Energy`s Office of Environmental Restoration and Waste Management (EM) is responsible for remediating its contaminated sites and managing its waste inventory in a safe and efficient manner. EM`s Office of Technology Development (OTD) supports applied research and demonstration efforts to develop and transfer innovative, cost-effective technologies to its site clean-up and waste management programs within EM`s Office of Environmental Restoration and Office of Waste Management. The purpose of the Technology Catalogue is to provide performance data on OTD-developed technologies to scientists and engineers assessing and recommending technical solutions within the Department`s clean-up and waste management programs, asmore » well as to industry, other federal and state agencies, and the academic community. OTD`s applied research and demonstration activities are conducted in programs referred to as Integrated Demonstrations (IDs) and Integrated Programs (IPs).     In this report, we start by examining previous efforts at linking science and DOE EM research with cleanup activities. Many of these efforts were initiated by creating science and technology roadmaps. A recurring feature of successfully implementing these roadmaps into EM applied research efforts and successful cleanup is the focus on integration. Such integration takes many forms, ranging from combining information generated by various scientific disciplines, to providing technical expertise to facilitate successful application of novel technology, to bringing the resources and creativity of many to address the common goal of moving EM cleanup forward. Successful projects identify and focusmore » research efforts on addressing the problems and challenges that are causing “failure” in actual cleanup activities. In this way, basic and applied science resources are used strategically to address the particular unknowns that are barriers to cleanup. The brief descriptions of the Office of Science basic (Environmental Remediation Science Program [ERSP]) and EM’s applied (Groundwater and Soil Remediation Program) research programs in subsurface science provide context to the five “crosscutting” themes that have been developed in this strategic planning effort. To address these challenges and opportunities, a tiered systematic approach is proposed that leverages basic science investments with new applied research investments from the DOE Office of Engineering and Technology within the framework of the identified basic science and applied research crosscutting themes. These themes are evident in the initial portfolio of initiatives in the EM groundwater and soil cleanup multi-year program plan. As stated in a companion document for tank waste processing (Bredt et al. 2008), in addition to achieving its mission, DOE EM is experiencing a fundamental shift in philosophy from driving to closure to enabling the long-term needs of DOE and the nation.« less

    Emergency management (EM) and business continuity management (BCM) frameworks incorporate various strategic and operational measures. Defined within a number of national and international standards and guidelines, such concepts may be integrated within one another to provide increased resilience to disruptive events. Nevertheless, there is a degree of dispute regarding concept integration among security and EM professionals and bodies of knowledge. In line with cognitive psychology exemplar-based concepts, such disputes may be associated with a lack of precision in communality in the approach to EM and BCM. This paper presents a two-stage study, where stage 1 critiqued national and international literature and stage 2 applied semi-structured interviews with security managers in Western Australia. Findings indicate the existence of contradictory views on EM and its integration within BCM. As such, this study concludes that EM is considered a vital component of BCM by the majority of security managers. However, there is broader dispute regarding its degree of integration. Understanding the underpinnings of such disputes will aid in raising the standards and application of professionalism within security, EM and BCM domains, supporting clarification and definition of professional boundaries.

    Public participation in Office of Environmental Management (EM) activities throughout the DOE complex is a critical component of the overall success of remediation and waste management efforts. The challenges facing EM and its stakeholders over the next decade or more are daunting (Nuclear Waste News 1996). Achieving a mission composed of such challenges will require innovation, dedication, and a significant degree of good will among all stakeholders. EM's efforts to date, including obtaining and using inputs offered by EM stakeholders, have been notable. Public participation specialists have accepted and met challenges and have consistently tried to improve their performance. Theymore » have reported their experiences both formally and informally (e.g., at professional conferences and EM Public Participation Network Workshops, other internal meetings of DOE and contractor public participation specialists, and one-on-one consultations) in order to advance the state of their practice. Our research, and our field research in particular (including our interactions with many representatives of numerous stakeholder groups at nine DOE sites with diverse EM problems), have shown that it, is possible to develop coherent results even in a problem domain as complex as that of EM. We conclude that performance-based evaluations of public participation appear possible, and we have recommended an approach, based on combined and integrated multi-stakeholder views on the attributes of successful public participation and associated performance indicators, that seems workable and should be acceptable to diverse stakeholders. Of course, as an untested recommendation, our approach needs the validation that can only be achieved by application (perhaps at a few DOE sites with ongoing EM activities). Such an application would serve to refine the proposed approach in terms of its clarity, its workability, and its potential for full-scale use by EM and, potentially, other government agencies and private sector concerns.« less

Cassini' 1.5m telescope of the Astronomical Observatory of Bologna located in Loiano (Italy).

    Protein-protein interactions carry out a large variety of essential cellular processes. Cryo-electron microscopy (cryo-EM) is a powerful technique for the modeling of protein-protein interactions at a wide range of resolutions, and recent developments have caused a revolution in the field. At low resolution, cryo-EM maps can drive integrative modeling of the interaction, assembling existing structures into the map. Other experimental techniques can provide information on the interface or on the contacts between the monomers in the complex. This inevitably raises the question regarding which type of data is best suited to drive integrative modeling approaches. Systematic comparison of the prediction accuracy and specificity of the different integrative modeling paradigms is unavailable to date. Here, we compare EM-driven, interface-driven, and contact-driven integrative modeling paradigms. Models were generated for the protein docking benchmark using the ATTRACT docking engine and evaluated using the CAPRI two-star criterion. At 20 Å resolution, EM-driven modeling achieved a success rate of 100%, outperforming the other paradigms even with perfect interface and contact information. Therefore, even very low resolution cryo-EM data is superior in predicting heterodimeric and heterotrimeric protein assemblies. Our study demonstrates that a force field is not necessary, cryo-EM data alone is sufficient to accurately guide the monomers into place. The resulting rigid models successfully identify regions of conformational change, opening up perspectives for targeted flexible remodeling. Copyright © 2016 Biophysical Society.
