    During July through August 2014, Operation Protective Edge, a military conflict between Israel and the Hamas regime in Gaza, dramatically affected both populations. Magen David Adom (MDA), the Israeli national Emergency Medical Service (EMS) and a member of the Red Cross, faced a unique challenge during the conflict: to continue providing crucial service to the entire civilian population of Israel, which was under constant missile threat. This challenge included not only providing immediate care for routine EMS calls under missile threat, but also preparing and delivering immediate care to civilians injured in attacks on major cities, as well as small communities, in Israel. This task is a challenge for a civilian EMS agency that normally operates in a non-military environment, yet, in an instant, must enhance its capability to respond to a considerable threat to its population. During Operation Protective Edge, MDA provided care for 842 wounded civilians and utilized a significant amount of its resources. Providing EMS services for a civilian population in a mixed civilian/military scenario is a challenging task on a national level for an EMS system, especially when the threat lasts for weeks. This report describes MDA's preparedness and operations during Operation Protective Edge, and the unique EMS challenges and dilemmas the agency faced.

    In 1995, a Society for Academic Emergency Medicine in-service survey reported high rates of verbal and physical abuse experienced by Emergency Medicine (EM) residents. We sought to determine the prevalence of abuse and harassment 10 years later to bring attention to these issues and determine if there has been a change in the prevalence of abuse over this time period. To determine the prevalence of abuse and harassment in a sample of EM residencies. We conducted a cross-section survey of EM residents from 10 residencies. EM residents were asked about their experience with verbal abuse, verbal threats, physical threats, physical attacks, sexual harassment, and racial harassment; and by whom. The primary outcome of the study was the prevalence of abuse and harassment as reported by EM residents. There were 196 of 380 residents (52%) who completed the survey. The prevalence of any type of abuse experienced was 91%; 86% of residents experienced verbal abuse, 65% verbal threats, 50% physical threats, 26% physical attacks, 23% sexual harassment, and 26% racial harassment. Women were more likely than men to encounter sexual harassment (37% [38/102] vs. 8% [7/92]; p < 0.001). Racial harassment was not limited to minorities (23% [16/60] for Caucasians vs. 26% [29/126] for non-Caucasians; p = 0.59). Senior residents were more likely to have encountered verbal and physical abuse. Only 12% of residents formally reported the abuse they experienced.     Fire ants and AHBs are aggressive social insects that readily attack when they perceive a threat. Rarely, each can cause fatal allergic reactions. However, AHBs can overwhelm and kill even healthy, nonallergic people. Such encounters are relatively rare, yet increasing in frequency. As a rule, fire ants can't overwhelm a healthy, mobile person, and even hundreds of stings are rarely fatal. AHBs, however, pose a greater threat to EMS and fire personnel when encountered. Every EMS agency in areas where AHBs are located (or predicted to migrate) should develop protocols for AHB attacks. Such a response should assure adequate personnel and, above all, rescuer safety. The protocol should be periodically practiced and reviewed.

    The Environmental Management System (EMS) is commonly implemented in private firms. However, on the basis of a strengths, weaknesses, opportunities and threats (SWOT) analysis, our work analyzes the consequences of implementing an EMS within the context of local public administrations, particularly regarding the City Council of Ohanes in Almería (Spain). This is the first European corporation to implement an EMS according to the ISO 14001 Standard, certified by the Spanish Association of Normalization and Certification. Its analysis would be equivalent to the Shumpeterian "market innovator study", so that public administration "followers" can take advantage of the derived benefits and of minimizing the negative effects of such an experience. On the other hand, we show that the economic and environmental advantages derived from the EMS go beyond the activities that the City Council is in charge of. They have spillover effects that extend them to all economic activities in the municipality and these effects are expected to be increased in the medium and long-term perspective. In this paper, we compare the costs and benefits that the municipality obtains in two cases: the City Council implements the EMS or it does not implement it. The main objective of this article is to show the economic and environmental advantages obtained by a municipality when it is only the City Council who is implementing an EMS. It is logical to suppose that this case study can stimulate other municipalities to use this instrument, even if the economic and environmental characteristics of the municipality are different.

    Publication bias occurs when the published research results are systematically unrepresentative of the population of studies that have been conducted, and is a potential threat to meaningful meta-analysis. The Copas selection model provides a flexible framework for correcting estimates and offers considerable insight into the publication bias. However, maximizing the observed likelihood under the Copas selection model is challenging because the observed data contain very little information on the latent variable. In this article, we study a Copas-like selection model and propose an expectation-maximization (EM) algorithm for estimation based on the full likelihood. Empirical simulation studies show that the EM algorithm and its associated inferential procedure performs well and avoids the non-convergence problem when maximizing the observed likelihood. © The Author 2017.     The overarching mission of prehospital Emergency Medical Services (EMS) is to deliver life-saving care for people when their needs are greatest. Fulfilling this mission is challenged by threats to patient and provider safety. The EMS setting is high-risk because care is delivered rapidly in the out-of-hospital setting where patient-benefiting resources are limited. There is growing evidence that safety culture varies widely across EMS agencies. A poor safety culture may manifest as error in medication, back injuries, and other poor outcomes for patient and provider. Recently, federal and national leaders of EMS (i.e., the National Highway Traffic Safety Administration) have made improving EMS safety culture a national priority. Unfortunately, there are few initiatives that can help local EMS leaders achieve that priority. We describe the successful EMS Champs Fellowship program supported by the Jewish Healthcare Foundation (JHF) designed to train EMS leaders to improve safety for patients and providers. PMID:23150883

    Discipline-based science concept assessments are powerful tools to measure learners' disciplinary core ideas. Among many such assessments, the Brief Electricity and Magnetism Assessment (BEMA) has been broadly used to gauge student conceptions of key electricity and magnetism (E&M) topics in college-level introductory physics courses. Differing from typical concept inventories that focus only on one topic of a subject area, BEMA covers a broad range of topics in the electromagnetism domain. In spite of this fact, prior studies exclusively used a single aggregate score to represent individual students' overall understanding of E&M without explicating the construct of this assessment. Additionally, BEMA has been used to compare traditional physics courses with a reformed course entitled Matter and Interactions (M&I). While prior findings were in favor of M&I, no empirical evidence was sought to rule out possible differential functioning of BEMA that may have inadvertently advantaged M&I students. In this study, we used Rasch analysis to seek two missing pieces regarding the construct and differential functioning of BEMA. Results suggest that although BEMA items generally can function together to measure the same construct of application and analysis of E&M concepts, several items may need further revision. Additionally, items that demonstrate differential functioning for the two courses are detected. Issues such as item contextual features and student familiarity with question settings may underlie these findings. This study highlights often overlooked threats in science concept assessments and provides an exemplar for using evidence-based reasoning to make valid inferences and arguments.

    Background On Friday March 11, 2011 a 9.0 magnitude earthquake triggered a tsunami off the eastern coast of Japan, resulting in thousands of lives lost and billions of dollars in damage around the Pacific Rim. The tsunami first reached the California coast on Friday, March 11th, causing more than $70 million in damage and at least one death. While the tsunami’s impact on California pales in comparison to the destruction caused in Japan and other areas of the Pacific, the event tested emergency responders’ ability to rapidly communicate and coordinate a response to a potential threat. Methods To evaluate the local public health system emergency response to the tsunami threat in California, we surveyed all local public health, emergency medical services (EMS), and emergency management agencies in coastal or floodplain counties about several domains related to the tsunami threat in California, including: (1) the extent to which their community was affected by the tsunami, (2) when and how they received notification of the event, (3) which public health response activities were carried out to address the tsunami threat in their community, and (4) which organizations contributed to the response. Public health activities were characterized using the Centers for Disease Control and Prevention (CDC) Public Health Preparedness Capabilities (PHEP) framework. Findings The tsunami's impact on coastal communities in California ranged widely, both in terms of the economic consequences and the response activities. Based on estimates from the National Oceanic and Atmospheric Administration (NOAA), ten jurisdictions in California reported tsunami-related damage, which ranged from $15,000 to $35 million. Respondents first became aware of the tsunami threat in California between the hours of 10:00pm Pacific Standard Time (PST) on Thursday March 10th and 2:00pm PST on Friday March 11th, a range of 16 hours, with notification occurring through both formal and informal channels. In response to this threat, the activities most commonly reported by the local government agencies included in this study were: emergency public information and warning, emergency operations coordination, and inter-organizational information sharing, which were reported by 86%, 75%, and 65% of all respondents, respectively. When looking at the distribution of responsibility, emergency management agencies were the most likely to report assuming a lead role in these common activities as well as those related to evacuation and community recovery. While activated less frequently, public health agencies carried out emergency response functions related to surveillance and epidemiology, environmental health, and mental health/psychological support. Both local public health and EMS agencies took part in mass care and medical material management activities. A large network of organizations contributed to response activities, with emergency management, law enforcement, fire, public health, public works, EMS, and media cited by more than half of respondents. Conclusions In response to the tsunami threat in California, we found that emergency management agencies assumed a lead role in the local response efforts. While public health and medical agencies played a supporting role in the response, they uniquely contributed to a number of specific activities.     Introduction Based upon military experience, law enforcement has developed guidelines for medical care during high-threat conditions. The purpose of the current study was to provide a descriptive analysis of reported outcomes of law enforcement medical interventions. This was a descriptive analysis of a convenience sample of cases submitted to the Wisconsin Tactical Medicine Initiative (Wisconsin USA), after the provision of successful patient care, between January 2010 and December 2015. The study was reviewed by the Mayo Foundation Institutional Review Board (Rochester, Minnesota USA) and deemed exempt. Nineteen agencies submitted information during the study period. Of the 56 episodes of care reported, four (7.1%) cases involved care provided to injured officers while 52 (92.9%) involved care to injured civilians, including suspects. In at least two cases, on-going threats existed during the provision of medical care to an injured civilian. Law enforcement rendered care prior to Emergency Medical Services (EMS) arrival in all but two cases. The current case series demonstrates the life-saving potential for law enforcement personnel trained and equipped under current Tactical Combat Casualty Care (TCCC)/ Committee on Tactical Emergency Casualty Care (C-TECC) tactical casualty care guidelines. Although originally developed to save the lives of wounded combat personnel, in the civilian sector, the training appears more likely to save victims rather than law enforcement personnel.     Abstract Those who are high in external motivation to respond without prejudice (EMS) tend to focus on non-racial attributes when describing others. This fMRI study examined the neural processing of race and an alternative yet stereotypically relevant attribute (viz., socioeconomic status: SES) as a function of the perceiver’s EMS. Sixty-one White participants privately formed impressions of Black and White faces ascribed with high or low SES. Analyses focused on regions supporting race- and status-based reward/salience (NAcc), evaluation (VMPFC) and threat/relevance (amygdala). Consistent with previous findings from the literature on status-based evaluation, we observed greater neural responses to high-status (vs low-status) targets in all regions of interest when participants were relatively low in EMS. In contrast, we observed the opposite pattern when participants were relatively high in EMS. Notably, all effects were independent of target race. In summary, White perceivers’ race-related motivations similarly altered their neural responses to the SES of Black and White targets. Specifically, the findings suggest that EMS may attenuate the positive value and/or salience of high status in a mixed-race context. Findings are discussed in the context of the stereotypic relationship between race and SES. PMID:29077925

  11. The threat of funding cuts for graduate medical education: survey of decision makers.

    To assess the potential actions of medical school deans, graduate medical education (GME) committee chairs, and hospital chief executive officers (CEOs) regarding future funding reductions for residency training. Specifically, institutions with emergency medicine (EM) residencies were surveyed to see whether EM training was disproportionally at risk for reductions. An anonymous 2-page survey was used. Ninety-eight EM residency programs were identified using the American Medical Association Graduate Medical Education Directory 1994-95. Seventy deans, 102 GME chairs, and 97 hospital CEOs were identified. The survey posed a hypothetical 25% forced reduction in residency positions and asked the decision makers for their responses. Options included: 1) proportional reductions of training positions from all residencies, 2) proportional reductions in either primary care or specialty residency positions, or 3) reduction or elimination of specific training programs. The survey asked for a first and second choice of residencies to be reduced or eliminated from an alphabetical list of 17. The survey elicited explanations for each program reduction. 200 (74%) of 269 surveys were returned. Eighty-four responders selected specific residencies to be reduced or eliminated. EM was selected 8 times, making EM the seventh most vulnerable residency to be targeted for reductions. The decision makers who selected proportional reductions chose to reduce across all residencies 32 times, among only the specialty residencies 129 times, and among only the primary care residencies 3 times. In the setting of anticipated residency cuts, favored proportional reductions in specialty residencies would likely affect EM training. However, most GME decision makers with an existing EM residency program do not consider the EM residency a top choice to be reduced or eliminated.

    Eye Movement Desensitization and Reprocessing (EMDR) therapy for posttraumatic stress disorder involves making eye movements (EMs) during recall of a traumatic image. Experimental studies have shown that the dual task decreases self-reported memory vividness and emotionality. However valuable, these data are prone to demand effects and little can be inferred about the mechanism(s) underlying the observed effects. The current research aimed to fill this lacuna by providing two objective tests of memory performance. Experiment I involved a stimulus discrimination task. Findings were that EM during stimulus recall not only reduces self-reported memory vividness, but also slows down reaction time in a task that requires participants to discriminate the stimulus from perceptually similar stimuli. Experiment II involved a fear conditioning paradigm. It was shown that EM during recall of a threatening stimulus intensifies fearful responding to a perceptually similar yet non-threat-related stimulus, as evidenced by increases in danger expectancies and skin conductance responses. The latter result was not corroborated by startle EMG data. Together, the findings suggest that the EM manipulation renders stimulus attributes less accessible for future recall. It has to be organised in such a way as to include all necessary services—such as triage accurate initial assessment, prompt resuscitation, efficient management of emergency cases, and transport to definitive care. The global economic downturn has had a direct effect on the health sector and poses additional threats to the healthcare system. Greece is one of the hardest-hit countries. This manuscript aims to present the structure of the Greek EMS system and the impact of the current economic recession on it. Nowadays, primary care suffers major shortages in crucial equipment, unmet health needs, and ineffective central coordination. Patients are also facing economic limitations that lead to difficulties in using healthcare services. The multi-factorial problem of in-hospital EMS overcrowding is also evident and has been linked with potentially poorer clinical outcomes. Furthermore, the ongoing refugee crisis challenges the national EMS. Adoption of a triage scale, expansion of the primary care network, and an effective primary–hospital continuum of care are urgently needed in Greece to provide comprehensive, culturally competent, and high-quality health care.     ABSTRACT Human papillomavirus 16 (HPV16) is a worldwide health threat and an etiologic agent of cervical cancer. To understand the antigenic properties of HPV16, we pursued a structural study to elucidate HPV capsids and antibody interactions. The cryo-electron microscopy (cryo-EM) structures of a mature HPV16 particle and an altered capsid particle were solved individually and as complexes with fragment of antibody (Fab) from the neutralizing antibody H16.V5. Fitted crystal structures provided a pseudoatomic model of the virus-Fab complex, which identified a precise footprint of H16.V5, including previously unrecognized residues. The altered-capsid–Fab complex map showed that binding of the Fab induced significant conformational changes that were not seen in the altered-capsid structure alone. These changes included more ordered surface loops, consolidated so-called “invading-arm” structures, and tighter intercapsomeric connections at the capsid floor. The H16.V5 Fab preferentially bound hexavalent capsomers likely with a stabilizing effect that directly correlated with the number of bound Fabs. Additional cryo-EM reconstructions of the virus-Fab complex for different incubation times and structural analysis provide a model for a hyperstabilization of the capsomer by H16.V5 Fab and showed that the Fab distinguishes subtle differences between antigenic sites. IMPORTANCE Our analysis of the cryo-EM reconstructions of the HPV16 capsids and virus-Fab complexes has identified the entire HPV.V5 conformational epitope and demonstrated a detailed neutralization mechanism of this clinically important monoclonal antibody against HPV16. The Fab bound and ordered the apical loops of HPV16. This conformational change was transmitted to the lower region of the capsomer, resulting in enhanced intercapsomeric interactions evidenced by the more ordered capsid floor and “invading-arm” structures. This study advances the understanding of the neutralization mechanism used by H16.V5. PMID:25392224

    Situation analysis is defined as a process, the examination of a situation, its elements, and their relations, to provide and maintain a product, i.e., a state of situation awareness, for the decision maker. Data fusion is a key enabler to meeting the demanding requirements of military situation analysis support systems. According to the data fusion model maintained by the Joint Directors of Laboratories' Data Fusion Group, impact assessment estimates the effects on situations of planned or estimated/predicted actions by the participants, including interactions between action plans of multiple players. In this framework, the appraisal of actual or potential threats is a necessary capability for impact assessment. This paper reviews and discusses in details the fundamental concepts of threat analysis. In particular, threat analysis generally attempts to compute some threat value, for the individual tracks, that estimates the degree of severity with which engagement events will potentially occur. Presenting relevant tracks to the decision maker in some threat list, sorted from the most threatening to the least, is clearly in-line with the cognitive demands associated with threat evaluation. A key parameter in many threat value evaluation techniques is the Closest Point of Approach (CPA). Along this line of thought, threatening tracks are often prioritized based upon which ones will reach their CPA first. Hence, the Time-to-CPA (TCPA), i.e., the time it will take for a track to reach its CPA, is also a key factor. Unfortunately, a typical assumption for the computation of the CPA/TCPA parameters is that the track velocity will remain constant. When a track is maneuvering, the CPA/TCPA values will change accordingly. These changes will in turn impact the threat value computations and, ultimately, the resulting threat list. This is clearly undesirable from a command decision-making perspective. In this regard, the paper briefly discusses threat value stabilization approaches based on neural networks and other mathematical techniques.

    LTAS is a totally integrated modeling and simulation environment designed for the purpose of ascertaining the susceptibility of Air Force pilots and air crews to optical radiation threats. Using LTAS, mission planners can assess the operational impact of optically directed energy weapons and countermeasures. Through various scenarios, threat analysts are able to determine the capability of laser threats and their impact on operational missions including the air crew's ability to complete their mission effectively. Additionally, LTAS allows the risk of laser use on training ranges and the requirement for laser protection to be evaluated. LTAS gives mission planners and threat analysts complete control of the threat environment including threat parameter control and placement, terrain mapping (line-of-site), atmospheric conditions, and laser eye protection (LEP) selection. This report summarizes the design of the final version of LTAS, and the modeling methodologies implemented to accomplish analysis.

This study summarizes the abilities and limitations of real-time PCR, electron microscopy (EM), and virus isolation when used to detect potential bioweapons. In particular, we investigated the inhibitory influences of different common household products present in environmental specimens on PCR yield, EM detection, and virus isolation. We used vaccinia virus as a model for orthopoxviruses by spiking it into specimens. In the second part of the study, we describe modifications of diagnostic methods to overcome inhibitory effects. A variety of PCR amplification enhancers, DNA extraction protocols, and applications of internal controls were evaluated to improve diagnostic simplicity, speed, and reliability. As a result, we strongly recommend using at least two different frontline techniques in parallel, e.g., EM and PCR. A positive result obtained by any one of these techniques should be followed by a biological method to confirm the putative diagnosis. Confirmatory methods include virus isolation followed by an agent-specific immunofluorescence assay to confirm the presence of replication-competent particles.

    A critical component of aviation security consists of screening passengers and baggage to protect airports and aircraft from terrorist threats. Advancements in screening device technology have increased the ability to detect these threats; however, specifying the operational configurations of these devices in response to changes in the threat environment can become difficult. This article proposes to use Fisher information as a statistical measure for detecting changes in the threat environment. The perceived risk of passengers, according to prescreening information and behavior analysis, is analyzed as the passengers sequentially enter the security checkpoint. The alarm responses from the devices used to detect threats are also analyzed to monitor significant changes in the frequency of threat items uncovered. The key results are that this information-based measure can be used within the Homeland Security Advisory System to indicate changes in threat conditions in real time, and provide the flexibility of security screening detection devices to responsively and automatically adapt operational configurations to these changing threat conditions. © 2012 Society for Risk Analysis.     The Florida manatee (Trichechus manatus latirostris) is an endangered marine mammal endemic to the southeastern United States. The primary threats to manatee populations are collisions with watercraft and the potential loss of warm-water refuges. For the purposes of listing, recovery, and regulation under the Endangered Species Act (ESA), an understanding of the relative effects of the principal threats is needed. This work is a quantitative approach to threats analysis, grounded in the assumption that an appropriate measure of status under the ESA is based on the risk of extinction, as quantified by the probability of quasi-extinction. This is related to the qualitative threats analyses that are more common under the ESA, but provides an additional level of rigor, objectivity, and integration. In this approach, our philosophy is that analysis of the five threat factors described in Section 4(a)(1) of the ESA can be undertaken within an integrated quantitative framework. The basis of this threats analysis is a comparative population viability analysis. This involves forecasting the Florida manatee population under different scenarios regarding the presence of threats, while accounting for process variation (environmental, demographic, and catastrophic stochasticity) as well as parametric and structural uncertainty. We used the manatee core biological model (CBM) for this viability analysis, and considered the role of five threats: watercraft-related mortality, loss of warm-water habitat in winter, mortality in water-control structures, entanglement, and red tide. All scenarios were run with an underlying parallel structure that allowed a more powerful estimation of the effects of the various threats. The results reflect our understanding of manatee ecology (as captured in the structure of the CBM), our estimates of manatee demography (as described by the parameters in the model), and our characterization of the mechanisms by which the threats act on manatees. As an example of the type of results generated, we estimated that the probability of the manatee population falling to less than 250 adults on either the Atlantic or Gulf coasts (from a current statewide population size of near 3300) within 100 years is 8.6%. Complete removal of the watercraft threat alone would reduce this risk to 0.4%; complete removal of the warm-water threat to 4.2%; removal of both threats would reduce the risk to 0.1%. The modeling approach we have taken also allows us to consider partial removal of threats, as well as removal of multiple threats simultaneously. We believe the measure we have proposed (probability of quasi-extinction over y years, with quasi-extinction defined as dropping below a threshold of z on either coast) is a suitable measure of status that integrates a number of the elements that are relevant to interpretation under the ESA (it directly integrates risk of extinction and reduction of range, and indirectly integrates loss of genetic diversity). But the identification of the time frame of interest and the tolerable risk of quasi-extinction are policy decisions, and an ecology-based quasi-extinction threshold has not yet been determined. We have endeavored to provide results over a wide range of these parameters to give decision-makers useful information to assess status. This assessment of threats suggests that watercraft-related mortality is having the greatest impact on manatee population growth and resilience. Elimination of this single threat would greatly reduce the probability of quasi-extinction. Loss of warm-water is also a significant threat, particularly over the long-term. Red tide and entanglement, while noticeable threats, have had less of an impact on the manatee population. The effect of water control structures may have already been largely mitigated.     Elementary modes (EMs) are steady-state metabolic flux vectors with minimal set of active reactions. Each EM corresponds to a metabolic pathway. Therefore, studying EMs is helpful for analyzing the production of biotechnologically important metabolites. However, memory requirements for computing EMs may hamper their applicability as, in most genome-scale metabolic models, no EM can be computed due to running out of memory. In this study, we present a method for computing randomly sampled EMs. In this approach, a network reduction algorithm is used for EM computation, which is based on flux balance-based methods. We show that this approach can be used to recover the EMs in the medium- and genome-scale metabolic network models, while the EMs are sampled in an unbiased way. The applicability of such results is shown by computing “estimated” control-effective flux values in Escherichia coli metabolic network.

    Stereotype threat as an example of serious interpersonal strain at workplace can lead either to impaired work engagement or it can motivate workers to strengthen their efforts to disconfirm a stereotype and can result in excessive work engagement. Thus, the basic aim of the study was to examine whether stereotype threat is related to burnout or to work engagement. The mediating role of the negative and positive emotions were also tested in the classical approach. Mediational analysis revealed a linear relation of stereotype threat and burnout, mediated by negative emotions and a quadratic relationship between stereotype threat and work engagement. In the latter analysis none of the mediators were significant. Therefore, the results showed that both burnout and work engagement are associated with stereotype threat at the workplace, probably depending on the stage of response to the stereotype threat. Further research should confirm these associations in a longitudinal study.
