The U.S. Environmental Protection Agency promulgated national emission standards for emissions of radionuclides other than radon from US Department of Energy facilities in Chapter 40 of the Code of Federal Regulations (CFR) 61, Subpart H. This regulatory standard limits the annual effective dose that any member of the public can receive from Department of Energy facilities to 0.1 mSv. Lawrence Livermore National Security, LLC operates facilities at Lawrence Livermore National Laboratory (LLNL) in which radionuclides are handled and stored. These facilities are subject to the U.S. Environmental Protection Agency (EPA) National Emission Standards for Hazardous Air Pollutants (NESHAPs) in Code of Federal Regulations (CFR) Title 40, Part 61, Subpart H, which regulates radionuclide emissions to air from Department of Energy (DOE) facilities. Subpart FF of 40 CFR Part 61 addresses benzene emissions from waste operations at petroleum refineries, chemical manufacturing plants, coke by-product plants, and waste management units that manage wastes from these facilities. Subpart FF, also known as the benzene waste operations national emission standards for hazardous air pollutants (NESHAP), was amended and published in the Federal Register on January 7, 1993. The U.S. Department of Energy, National Nuclear Security Administration Nevada Site Office operates the Nevada National Security Site (NNSS, formerly the Nevada Test Site) and North Las Vegas Facility (NLVF). From 1951 through 1992, the NNSS was the continental testing location for U.S. nuclear weapons. The release of radionuclides from NNSS activities has been monitored since the initiation of atmospheric testing. Limitation to underground detonations after 1962 greatly reduced radiation exposure to the public surrounding the NNSS. After nuclear testing ended in 1992, NNSS radiation monitoring focused on detecting airborne radionuclides from historically contaminated soils. These radionuclides are derived frommore » re-suspension of soil (primarily by wind) and emission of tritium-contaminated soil moisture through evapotranspiration. Low amounts of tritium are also emitted to air at the NLVF, an NNSS support complex in North Las Vegas. To protect the public from harmful levels of man-made radiation, the Clean Air Act, National Emission Standards for Hazardous Air Pollutants (NESHAP) (Title 40 Code of Federal Regulations [CFR] Part 61 Subpart H) (CFR, 2010a) limits the release of radioactivity from a U.S. Department of Energy (DOE) facility to that which would cause 10 millirem per year (mrem/yr) effective dose equivalent to any member of the public. This limit does not include radiation unrelated to NNSS activities. Unrelated doses could come from naturally occurring radioactive elements, from sources such as medically or commercially used radionuclides, or from sources outside of the United States, such as those from the damaged Fukushima nuclear power plant in Japan. Because this report is intended to discuss radioactive air emissions during calendar year 2010, data on radionuclides in air from the 2011 Fukushima nuclear power plant releases are not presented but will be included in the report for calendar year 2011. The NNSS demonstrates compliance with the NESHAP limit by using environmental measurements of radionuclide air concentrations at critical receptor locations (U.S. Environmental Protection Agency [EPA] and DOE, 1995). This method was approved by the EPA for use on the NNSS in 2001(EPA, 2001a) and has been the sole method used since 2005. Six locations on the NNSS have been established to act as critical receptor locations to demonstrate compliance with the NESHAP limit. These locations are actually pseudo-critical receptor stations, because no member of the public actually resides at these onsite locations. Compliance is demonstrated if the measured annual average concentration is less than the NESHAP Concentration Levels (CLs) for Environmental Compliance listed in 40 CFR 61, Appendix E, Table 2 (CFR, 2010a). For multiple radionuclides, compliance is demonstrated when the sum of the fractions (determined by dividing each radionuclide's concentration by its CL and then adding the fractions together) is less than 1.0. In 2010, the potential dose from radiological emissions to air, resulting from both current and past NNSS activities, at onsite compliance monitoring stations was well below the 10 mrem/yr dose limit. Air sampling data collected at all air monitoring stations had average concentrations of radioactivity that were a fraction of the CL values. Concentrations ranged from less than 1 percent to a maximum of 17 percent of the allowed NESHAP limit. Because the nearest member of the public resides about 20 kilometers from potential release points on the NNSS, dose to the public would be only a small fraction of that measured on the NNSS. The potential dose to the public from NLVF emissions was also very low at 0.000032 mrem/yr, more than 300,000 times lower than the 10 mrem/yr limit.« less

There appears to be no compelling public health protection reason for EPA`s promulgation of NESHAP regulations to control air emissions of radioactive materials from NRC-licensed facilities engaged in activities associated with the practice and development of nuclear medicine. The NRC`s existing regulations provide the necessary controls for protection and EPA`s regulations would only add burdensome reporting requirements at substantial cost to medical treatment and diagnosis. Availability of nuclear medicine practice could be impacted and advancements through research delayed.

...This action proposes amendments to the national emission standards for hazardous air pollutants (NESHAP) for mercury emissions from mercury cell chlor-alkali plants (Mercury Cell NESHAP). On June 11, 2008, EPA proposed amendments to this NESHAP in response to a petition for reconsideration filed by the Natural Resources Defense Council (NRDC). This action is a supplement to the June 11, 2008, proposal. Specifically, this action proposes two options for amending the NESHAP for mercury emissions from mercury cell chlor-alkali plants. The first option would require the elimination of mercury emissions and thus encourage the conversion to non-mercury technology. The second option would require the measures proposed in 2008. These measures, which included significant improvements in the work practices to reduce fugitive emissions from the cell room, would result in near-zero levels of mercury emissions while still allowing the mercury cell facilities to continue to operate. We are specifically requesting comment on which of these options is more appropriate, and may finalize either option or a combination of elements from them. In addition, this action proposes several amendments that would apply regardless of which option we select. These proposed amendments are provisions of the existing NESHAP that would apply to periods of startup, shutdown, and malfunction (SSM), and corrections to compliance errors in the currently effective rule.

Environmental regulations such as National Emission Standards for Hazardous Air Pollutants (NESHAPs) are drivers for the implementation of environmentally compliant methodologies in the manufacture of aerospace hardware. In 1995, the Environmental Protection Agency (EPA) promulgated the NESHAP for the Aerospace Manufacture and Rework (Aerospace NESHAP) industry. Affected facilities were to be in compliance by September 1998. Several aerospace manufacturing operations are regulated within the Aerospace NESHAP including Depainting operations. The National Aeronautics and Space Administration (NASA), EPA, and United States Air Force (USAF) combined resources to evaluate the performance of nine alternative depainting processes. The seven alternative depainting processes were: (1) Chemical stripping (non-methylene chloride); (2) Carbon Dioxide Blasting; (3) Xenon Flashlamp; (4) Carbon Dioxide Laser Stripping; (5) Plastic Media Blasting; (6) Sodium Bicarbonate Wet Stripping; and (7) Waterjet Blasting and Wheat Starch Blasting. All epoxy primer and polyurethane top coat system was applied to 2024-T3 clad and non-clad aluminum test specimens. Approximately 200 test specimens were evaluated in this study. Each coupon was subjected to three, four, or five complete depainting cycles. This paper discusses the conclusions from the study including the test protocol, test parameters, and achievable strip rates for the alternative depainting processes. Test data includes immersion corrosion testing, sandwich corrosion testing and hydrogen embrittlement testing for the non-methylene chloride chemical strippers. Additionally, the cumulative effect of the alternative depainting processes on the metallurgical integrity of the test substrate is addressed with the results from tensile and fatigue evaluations.
On December 20, 2002, the EPA proposed national emission standards for HAP emissions from lime manufacturing plants located at major source facilities (67 FR 78046). Summaries of the comments, and the EPA's responses, are presented in this BID.

This National Emission Standards for Hazardous Air Pollutants (NESHAP) Annual Report has been prepared in a format to comply with the reporting requirements of 40 CFR 61.94 and the April 5, 1995 Memorandum of Agreement (MOA) between the Department of Energy (DOE) and the Environmental Protection Agency (EPA). According to the EPA approved NESHAP Monitoring Plan for the Tonopah Test Range (TTR), 40 CFR 61, subpart H, and the MOA, no additional monitoring or measurements are required at TTR in order to demonstrate compliance with the NESHAP regulation.

The U.S. Environmental Protection Agency (EPA) requires the use of the model CAP88 to estimate the total effective dose (TED) to an offsite maximally exposed individual (MEI) for demonstrating compliance with 40 CFR 61, Subpart H: The National Emission Standards for Hazardous Air Pollutants (NESHAP) regulations. For NESHAP compliance at the Savannah River Site (SRS), the EPA, the U.S. Department of Energy (DOE), South Carolina's Department of Health and Environmental Control, and SRS approved a dose assessment method in 1991 that models all radiological emissions as if originating from a generalized center of site (COS) location at two allowable stack heights (0 m and 61 m). However, due to changes in SRS missions, radiological emissions are no longer evenly distributed about the COS. An area-specific simulation of the 2015 SRS radiological airborne emissions was conducted to compare to the current COS method. The results produced a slightly higher dose estimate (2.97 × 10 mSv vs. 2.22 × 10 mSv), marginally changed the overall MEI location, and noted that H-Area tritium emissions dominated the dose. Thus, an H-Area dose model was executed as a potential simplification of the area-specific simulation by adopting the COS methodology and modeling all site emissions from a single location in H-Area using six stack heights that reference stacks specific to the tritium production facilities within H-Area. This "H-Area Tritium Stacks" method produced a small increase in TED estimates (3.03 × 10 mSv vs. 2.97 × 10 mSv) when compared to the area-specific simulation. This suggests that the current COS method is still appropriate for demonstrating compliance with NESHAP regulations but that changing to the H-Area Tritium Stacks assessment method may now be a more appropriate representation of operations at SRS.

These proposed amendments to the National Emission Standards for Hazardous Air Pollutants (NESHAP) for Publicly Owned Treatment Works (POTW) result from the results of the residual risk and technology review of that source category.
This action promulgates national emission standards for hazardous air pollutants (NESHAP) for the pesticide active ingredient (PAI) production source category under section 112 of the Clean Air Act as amended (CAA or Act).

The Rad-NESHAP program, part of the Air Quality Compliance team of LANL’s Compliance Programs group (EPC-CP), and the Radiation Instrumentation & Calibration team, part of the Radiation Protection Services group (RP-SVS), frequently partner on issues relating to characterizing air flow streams. This memo documents the most recent example of this partnership, involving performance testing of sulfur hexafluoride detectors for use in stack gas mixing tests. Additionally, members of the Rad-NESHAP program performed a functional trending test on a pair of optical particle counters, comparing results from a non-calibrated instrument to a calibrated instrument. Prior to commissioning a new stack samplingmore » system, the ANSI Standard for stack sampling requires that the stack sample location must meet several criteria, including uniformity of tracer gas and aerosol mixing in the air stream. For these mix tests, tracer media (sulfur hexafluoride gas or liquid oil aerosol particles) are injected into the stack air stream and the resulting air concentrations are measured across the plane of the stack at the proposed sampling location. The coefficient of variation of these media concentrations must be under 20% when evaluated over the central 2/3 area of the stack or duct. The instruments which measure these air concentrations must be tested prior to the stack tests in order to ensure their linear response to varying air concentrations of either tracer gas or tracer aerosol. The instruments used in tracer gas and aerosol mix testing cannot be calibrated by the LANL Standards and Calibration Laboratory, so they would normally be sent off-site for factory calibration by the vendor. Operational requirements can prevent formal factory calibration of some instruments after they have been used in hazardous settings, e.g., within a radiological facility with potential airborne contamination. The performance tests described in this document are intended to demonstrate the reliable performance of the test instruments for the specific tests used in stack flow characterization.« less

Most U.S. Department of Energy (DOE) facilities with radiological airborne releases use the U.S. Environmental Protection Agency's (EPA) environmental dosimetry code CAP88-PC to demonstrate compliance with regulations in 40CFR61, subpart H [National Emission Standards for Hazardous Air Pollutants: Radiological (NESHAP)]. In 2015, EPA released Version 4 of CAP88-PC, which included significant modifications that improved usability and age-dependent dose coefficients and usage factors for six age groups (infant, 1 y, 5 y, 10 y, 15 y, and adult). However, EPA has not yet provided specific guidance on how to use these age-dependent factors. For demonstrating compliance with DOE public dose regulations, the Savannah River Site (SRS) recently changed from using the maximally exposed individual (MEI) concept (adult male) to the representative person concept (age- and gender-averaged reference person). In this study, dose comparisons are provided between the MEI and a SRS-specific representative person using the age-specific dose coefficients and usage factors in CAP88-PC V.4. Dose comparisons also are provided for each of the six age groups using five radionuclides of interest at SRS (tritium oxide, Cs, Sr, Pu, and I). In general, the total effective dose increases about 11% for the representative person as compared to the current NESHAP MEI because of the inclusion of the more radiosensitive age groups.

The Environmental Protection Agency (EPA) requires the use of the computer model CAP88-PC to estimate the total effective doses (TED) for demonstrating compliance with 40 CFR 61, Subpart H (EPA 2006), the National Emission Standards for Hazardous Air Pollutants (NESHAP) regulations. As such, CAP88 Version 4.0 was used to calculate the receptor dose due to routine atmospheric releases at the Savannah River Site (SRS). For estimation, NESHAP dose-release factors (DRFs) have been supplied to Environmental Compliance and Area Closure Projects (EC&ACP) for many years. DRFs represent the dose to a maximum receptor exposed to 1 Ci of a specified radionuclidemore » being released into the atmosphere. They are periodically updated to include changes in the CAP88 version, input parameter values, site meteorology, and location of the maximally exposed individual (MEI). In this report, the DRFs were calculated for potential radionuclide atmospheric releases from 13 SRS release points. The three potential onsite MEI locations to be evaluated are B-Area, Three Rivers Landfill (TRL), and Savannah River Ecology Lab Conference Center (SRELCC) with TRL’s onsite workers considered as members-of-the-public, and the potential future constructions of dormitories at SRELCC and Barracks at B-Area. Each MEI location was evaluated at a specified compass sector with different area to receptor distances and was conducted for both ground-level and elevated release points. The analysis makes use of area-specific meteorological data (Viner 2014). The resulting DRFs are compared to the 2014 NESHAP offsite MEI DRFs for three operational areas; A-Area, H-Area, and COS for a release rate of 1 Ci of tritium oxide at 0 ft. elevation. CAP88 was executed again using the 2016 NESHAP MEI release rates for 0 and 61 m stack heights to determine the radionuclide dose at TRL from the center-of-site (COS).« less

Actions taken and planned at four Lockheed Martin Corporation (LMC) facilities to comply with the Aerospace MACT Standard are reviewed. Many LMC sites have taken proactive steps to reduce emissions and implement low VOC coating technology. Significant administrative, facility, and material challenges remain to achieve compliance with the upcoming NESHAP and Control Technology Guideline (CTG) standards. The facilities discussed herein set up programs to develop and implement compliance strategies. These facilities manufacture military aircraft, missiles, satellites, rockets, and electronic guidance and communications systems. Some of the facilities are gearing up for new production lines subject to new source MACT standards.more » At this time the facilities are reviewing compliance status of all primers, topcoats, maskants and solvents subject to the standard. Facility personnel are searching for the most efficient methods of satisfying the recordkeeping, reporting and monitoring, sections of the standards while simultaneously preparing or reviewing their Title V permit applications. Facility decisions on paint booths are the next highest priority. Existing dry filter paint booths will be subject to the filtration standard for existing paint booths which requires the use of two-stage filters. Planned paint booths for the F-22 program, and other new booths must comply with the standard for new and rebuilt booths which requires three stage or HEPA filters. Facilities looking to replace existing water wash paint booths, and those required to retrofit the air handling equipment to accommodate the two-stage filters, are reviewing issues surrounding the rebuilt source definition.« less

Midwest Research Institute, under a cooperative agreement with the Air Pollution Prevention and Control Division of the U.S. Environmental Protection Agency's (EPA's) National Risk Management Research Laboratory, is conducting a study to identify wood furniture and cabinet manufacturing facilities that have converted to low-volatile organic compound/hazardous air pollutant (VOC/HAP) coatings and to develop case studies for those facilities. The case studies include: (1) a discussion of the types of products each facility manufactures; (2) the types of low-VOC/HAP coatings each facility is using; (3) problems encountered in converting to low-VOC/HAP coatings; (4) equipment changes that were required; (5) the costsmore » associated with the conversion process, including capital costs associated with equipment purchases, research and development costs, and operating costs such as operator training in new application techniques; (6) advantages/disadvantages of the low-VOC/HAP coatings; and (7) customer feedback on products finished with the low-VOC/HAP coatings. The primary goals of the project are (1) to demonstrate that low-VOC/HAP coatings can be used successfully by many wood furniture manufacturing facilities, and (2) to assist other wood furniture manufacturing facilities in their conversion to low-VOC/HAP coatings, in particular facilities that do not have the resources to devote to extensive coatings research. In the initial Asbestos NESHAP rule promulgated in 1973, a distinction was made between building materials that would readily release asbestos fibers when damaged or disturbed and those materials that were unlikely to result in significant fiber release. The terms friable and nonfriable were used to make this distinction. EPA has since determined that, if severely damaged, otherwise nonfriable materials can release significant amounts of asbestos fibers. On October 1, OSHA`s revised asbestos rules became law, and with them are many changes from the previous 1986 standard. The presumed asbestos-containing material (PACM) rule is one of the bigger changes in the revised standard. OSHA has declared that owners must presume that there are certain high-risk asbestos-containing materials (ACM) in facilities built prior to 1981, unless bulk sample results prove them to be nonasbestos. The impact of this provision forces companies to think carefully before presuming where asbestos is or where it is not. Companies must also heed the EPA Asbestos National Emission Standards for Hazardous Air Pollutantsmore » (NESHAP), which require inspections and bulk sampling to identify materials prior to renovation or demolition. Short and long term needs should be examined when analyzing how to comply with the PACM provision. There are four options available. Option 1: Ignore the standard and face potential enforcement fines. Option 2: Presume all materials in pre-1981 buildings contain asbestos and simply post additional warning signs. Option 3: Survey or resurvey the facilities to be in compliance with the PACM ruling. Option 4: Conduct more comprehensive surveys. Option 3 is discussed in some detail.« less

The release of mercury to the environment is of particular concern because of its volatility, persistence, and tendency to bioaccumulate. The recovery of mercury from end-box exhaust at chlor-alkali facilities is important to prevent release into the environment and reduce emissions as required by NESHAP (National Emission Standards for Hazardous Air Pollutants). A pilot-scale photocatalytic reactor packed with silica-titania composite (STC) pellets was tested at a chloralkali facility over a 3-month period. This pilot reactor treated up to 10 ft3/min (ACFM) of end-box exhaust and achieved 95% removal. The pilot reactor was able to maintain excellent removal efficiency even with large fluctuations in influent mercury concentration (400-1600 microg/ft3). The STC pellets were regenerated ex situ by regeneration with hydrochloric acid and performed similarly to virgin STC pellets when returned to service. On the basis of these promising results, two full-scale reactors with in situ regeneration capabilities were installed and operated. After optimization, these reactors performed similarly to the pilot reactor. A cost analysis was performed comparing the treatment costs (i.e., cost per pound of mercury removed) for sulfur-impregnated activated carbon and the STC system. The STC proved to be both technologically and economically feasible for this installation.

A steroids processing plant located in northeastern Puerto Rico emits a combined average of 342 lb/hr of hazardous air pollutants (HAPs) and volatile organic compounds (VOCs) from various process operations. The approach that this facility used to implement maximum achievable control technology (MACT) may assist others who must contend with MACT for pharmaceutical or related manufacturing facilities. Federal air regulations define MACT standards for stationary sources emitting any of 189 HAPs. The MACT standards detailed in the NESHAPs are characterized by industry and type of emission control system or technology. It is anticipated that the standard will require HAP reductionsmore » of approximately 95%. The steroid plant`s emissions include the following pollutant loadings: VOC/HAP Emission Rate (lb/hr): Methanol 92.0; Acetone 35.0; Methylene chloride 126.0; Chloroform 25.0; Ethyl acetate 56.0; Tetrahydrofuran 5.00; and 1,4-Dioxane 3.00. The facility`s existing carbon adsorption control system was nearing the end of its useful life, and the operators sought to install an air pollution control system capable of meeting MACT requirements for the pharmaceutical industry. Several stand-alone and hybrid control technologies were considered for replacement of the carbon adsorption system at the facility. This paper examines the following technologies: carbon adsorption, membrane separation, thermal oxidation, membrane separation-carbon adsorption, and condensation-carbon adsorption. Each control technology is described; the advantages and disadvantages of utilizing each technology for the steroid processing plant are examined; and capital and operating costs associated with the implementation of each technology are presented. AIR DISPERSION MODELING AT THE WASTE ISOLATION PILOT PLANT

One concern at the Waste Isolation Pilot Plant (WIPP) is the amount of alpha-emitting radionuclides or hazardous chemicals that can become airborne at the facility and reach the Exclusive Use Area boundary as the result of a release from the Waste Handling Building (WHB) or from the underground during waste emplacement operations. The WIPP Safety Analysis Report (SAR), WIPP RCRA Permit, and WIPP Emergency Preparedness Hazards Assessments include air dispersion calculations to address this issue. Meteorological conditions at the WIPP facility will dictate direction, speed, and dilution of a contaminant plume of respirable material due to chronic releases or duringmore » an accident. Due to the paucity of meteorological information at the WIPP site prior to September 1996, the Department of Energy (DOE) reports had to rely largely on unqualified climatic data from the site and neighboring Carlsbad, which is situated approximately 40 km (26 miles) to the west of the site. This report examines the validity of the DOE air dispersion calculations using new meteorological data measured and collected at the WIPP site since September 1996. The air dispersion calculations in this report include both chronic and acute releases. Chronic release calculations were conducted with the EPA-approved code, CAP88PC and the calculations showed that in order for a violation of 40 CFR61 (NESHAPS) to occur, approximately 15 mCi/yr of 239Pu would have to be released from the exhaust stack or from the WHB. This is an extremely high value. Hence, it is unlikely that NESHAPS would be violated. A site-specific air dispersion coefficient was evaluated for comparison with that used in acute dose calculations. The calculations presented in Section 3.2 and 3.3 show that one could expect a slightly less dispersive plume (larger air dispersion coefficient) given greater confidence in the meteorological data, i.e. 95% worst case meteorological conditions. Calculations show that dispersion will decrease slightly if a more stable wind class is assumed, where very little vertical mixing occurs. It is recommended that previous reports which used fixed values for calculating the air dispersion coefficient be updated to reflect the new meteorological data, such as the WIPP Safety Analysis Report and the WIPP Emergency Preparedness Hazards Assessment. It is also recommended that uncertainty be incorporated into the calculations so that a more meaningful assessment of risk during accidents can be achieved.« less

...The Environmental Protection Agency has submitted an information collection request (ICR), ``NESHAP for Stationary Combustion Turbines (40 CFR Part 63, Subpart YYYY) (Renewal)'' (EPA ICR No. 1967.05, OMB Control No. 2060-0540) to the Office of Management and Budget (OMB) for review and approval in accordance with the Paperwork Reduction Act (44 U.S.C. 3501 et seq). This is a proposed extension of the ICR, which is currently approved through September 30, 2013. Public comments were previously requested via the Federal Register (77 FR 63813) on October 17, 2012, during a 60-day comment period. This notice allows for an additional 30 days for public comments. A fuller description of the ICR is given below, including its estimated burden and cost to the public. An Agency may not conduct or sponsor and a person is not required to respond to a collection of information unless it displays a currently valid OMB control number.
This November 2003 memo indicates that size presses or size press alternative (SP/SPA), and on-machine coaters that apply sizing or water-based clays as a component of papermaking system are not subject to requirement of Subpart JJJJ.

The guidelines may be used as a reference for advanced planning or, once the emergency presents itself, to help ensure that, to the extent feasible and compatible with other emergency measures, all appropriate steps are taken to safely handle and dispose of all asbestos, while avoiding unnecessary exposures to asbestos. The guidelines provide information that may be helpful to EPA Regional offices and delegated NESHAP agencies that must respond to emergencies involving asbestos. 2011 Radioactive Materials Usage Survey for Unmonitored Point Sources

This report provides the results of the 2011 Radioactive Materials Usage Survey for Unmonitored Point Sources (RMUS), which was updated by the Environmental Protection (ENV) Division's Environmental Stewardship (ES) at Los Alamos National Laboratory (LANL). ES classifies LANL emission sources into one of four Tiers, based on the potential effective dose equivalent (PEDE) calculated for each point source. Detailed descriptions of these tiers are provided in Section 3. The usage survey is conducted annually; in odd-numbered years the survey addresses all monitored and unmonitored point sources and in even-numbered years it addresses all Tier III and various selected other sources.more » This graded approach was designed to ensure that the appropriate emphasis is placed on point sources that have higher potential emissions to the environment. For calendar year (CY) 2011, ES has divided the usage survey into two distinct reports, one covering the monitored point sources (to be completed later this year) and this report covering all unmonitored point sources. This usage survey includes the following release points: (1) all unmonitored sources identified in the 2010 usage survey, (2) any new release points identified through the new project review (NPR) process, and (3) other release points as designated by the Rad-NESHAP Team Leader. Data for all unmonitored point sources at LANL is stored in the survey files at ES. LANL uses this survey data to help demonstrate compliance with Clean Air Act radioactive air emissions regulations (40 CFR 61, Subpart H). The remainder of this introduction provides a brief description of the information contained in each section. Section 2 of this report describes the methods that were employed for gathering usage survey data and for calculating usage, emissions, and dose for these point sources. It also references the appropriate ES procedures for further information. Section 3 describes the RMUS and explains how the survey results are organized. The RMUS Interview Form with the attached RMUS Process Form(s) provides the radioactive materials survey data by technical area (TA) and building number. The survey data for each release point includes information such as: exhaust stack identification number, room number, radioactive material source type (i.e., potential source or future potential source of air emissions), radionuclide, usage (in curies) and usage basis, physical state (gas, liquid, particulate, solid, or custom), release fraction (from Appendix D to 40 CFR 61, Subpart H), and process descriptions. In addition, the interview form also calculates emissions (in curies), lists mrem/Ci factors, calculates PEDEs, and states the location of the critical receptor for that release point. [The critical receptor is the maximum exposed off-site member of the public, specific to each individual facility.] Each of these data fields is described in this section. The Tier classification of release points, which was first introduced with the 1999 usage survey, is also described in detail in this section. Section 4 includes a brief discussion of the dose estimate methodology, and includes a discussion of several release points of particular interest in the CY 2011 usage survey report. It also includes a table of the calculated PEDEs for each release point at its critical receptor. Section 5 describes ES's approach to Quality Assurance (QA) for the usage survey. Satisfactory completion of the survey requires that team members responsible for Rad-NESHAP (National Emissions Standard for Hazardous Air Pollutants) compliance accurately collect and process several types of information, including radioactive materials usage data, process information, and supporting information. They must also perform and document the QA reviews outlined in Section 5.2.6 (Process Verification and Peer Review) of ES-RN, 'Quality Assurance Project Plan for the Rad-NESHAP Compliance Project' to verify that all information is complete and correct.« less

This final rule modifies the Medicaid upper payment limits for inpatient hospital services, outpatient hospital services, nursing facility services, intermediate care facility services for the mentally retarded, and clinic services. For each type of Medicaid inpatient service, existing regulations place an upper limit on overall aggregate payments to all facilities and a separate aggregate upper limit on payments made to State-operated facilities. This final rule establishes an aggregate upper limit that applies to payments made to government facilities that are not State government-owned or operated, and a separate aggregate upper limit on payments made to privately-owned and operated facilities. This rule also eliminates the overall aggregate upper limit that had applied to these services. With respect to outpatient hospital and clinic services, this final rule establishes an aggregate upper limit on payments made to State government-owned or operated facilities, an aggregate upper limit on payments made to government facilities that are not State government-owned or operated, and an aggregate upper limit on payments made to privately-owned and operated facilities. These separate upper limits are necessary to ensure State Medicaid payment systems promote economy and efficiency. We are allowing a higher upper limit for payment to non-State public hospitals to recognize the higher costs of inpatient and outpatient services in public hospitals. In addition, to ensure continued beneficiary access to care and the ability of States to adjust to the changes in the upper payment limits, the final rule includes a transition period for States with approved rate enhancement State plan amendments.

Objective Injuries, trauma and non-communicable diseases are responsible for a rising proportion of death and disability in low-income and middle-income countries. Delivering effective emergency and urgent healthcare for these and other conditions in resource-limited settings is challenging. In this study, we sought to examine and characterise emergency and urgent care capacity in a resource-limited setting. Methods We conducted an assessment within all 30 primary and secondary hospitals and within a stratified random sampling of 30 dispensaries and health centres in western Kenya. The key informants were the most senior facility healthcare provider and manager available. Emergency physician researchers utilised a semistructured assessment tool, and data were analysed using descriptive statistics and thematic coding. Results No lower level facilities and 30% of higher level facilities reported having a defined, organised approach to trauma. 43% of higher level facilities had access to an anaesthetist. The majority of lower level facilities had suture and wound care supplies and gloves but typically lacked other basic trauma supplies. For cardiac care, 50% of higher level facilities had morphine, but a minority had functioning ECG, sublingual nitroglycerine or a defibrillator. Only 20% of lower level facilities had glucometers, and only 33% of higher level facilities could care for diabetic emergencies. No facilities had sepsis clinical guidelines. Conclusions Large gaps in essential emergency care capabilities were identified at all facility levels in western Kenya. There are great opportunities for a universally deployed basic emergency care package, an advanced emergency care package and facility designation scheme, and a reliable prehospital care transportation and communications system in resource-limited settings. The purposes of this study were to characterize the state of quality improvement (QI) in nursing facilities and to identify barriers to improvement from nursing leaders' perspectives. The study employed a non-experimental descriptive design, using closed- and open-ended survey questions in a sample of 51 nursing facilities in a midwestern state. Only two of these facilities had active QI programs. Furthermore, turnover and limited training among these nursing leaders represented major barriers to rapid implementation of such programs. This study is consistent with earlier findings that QI programs are limited in nursing homes.

Inpatient and skilled nursing facility (SNF) cost sharing in Medicare Advantage (MA) plans may reduce unnecessary use of these services. However, large out-of-pocket expenses potentially limit access to care and encourage beneficiaries at high risk of needing inpatient and postacute care to avoid or leave MA plans. In 2011 new federal regulations restricted inpatient and skilled nursing facility cost sharing and mandated limits on out-of-pocket spending in MA plans. After these regulations, MA members in plans with low premiums averaged $1,758 in expected out-of-pocket spending for an episode of seven hospital days and twenty skilled nursing facility days. Among members with the same low-premium plan in 2010 and 2011, 36 percent of members belonged to plans that added an out-of-pocket spending limit in 2011. However, these members also had a $293 increase in average cost sharing for an inpatient and skilled nursing facility episode, possibly to offset plans' expenses in financing out-of-pocket limits. Rapid diagnostic Tests (RDTs) for malaria enable diagnostic testing at primary care facilities in resource-limited settings, where weak infrastructure limits the use of microscopy. In 2010, Ghana adopted a test-before-treat guideline for malaria, with RDT use promoted to facilitate diagnosis. Yet healthcare practitioners still treat febrile patients without testing, or despite negative malaria test results. Few studies have explored RDT implementation beyond the notions of provider or patient acceptability. The aim of this study was to identify the factors directly influencing malaria RDT implementation at primary care facilities in a Ghanaian district. Qualitative interviews, focus groups and direct observations were conducted with 50 providers at six purposively selected primary care facilities in the Atwima-Nwabiagya district. Data were analysed thematically. RDT implementation was hampered by: (1) healthcare delivery constraints (weak supply chain, limited quality assurance and control, inadequate guideline emphasis, staffing limitations); (2) provider perceptions (entrenched case-management paradigms, limited preparedness for change); (3) social dynamics of care delivery (expected norms of provider-patient interaction, test affordability); and (4) limited provider engagement in policy processes leading to fragmented implementation of health sector reform. Limited health system capacity, socio-economic, political, and historical factors hampered malaria RDT implementation at primary care facilities in the study district. For effective RDT implementation providers must be: (1) adequately enabled through efficient allocation and management of essential healthcare commodities; (2) appropriately empowered with the requisite knowledge and skill through ongoing, effective professional development; and (3) actively engaged in policy dialogue to demystify socio-political misconceptions that hinder health sector reform policies from improving care delivery. Clear, consistent guideline emphasis, with complementary action to address deep-rooted provider concerns will build their confidence in, and promote uptake of recommended policies, practices, and technology for diagnosing malaria.
Improving Tanzanian childbirth service quality.

Purpose The purpose of this paper is to describe a quality improvement (QI) intervention in primary health facilities providing childbirth care in rural Southern Tanzania. Design/methodology/approach A QI collaborative model involving district managers and health facility staff was piloted for 6 months in 4 health facilities in Mtwara Rural district and implemented for 18 months in 23 primary health facilities in Ruangwa district. The model brings together healthcare providers from different health facilities in interactive workshops by: applying QI methods to generate and test change ideas in their own facilities; using local data to monitor improvement and decision making; and health facility supervision visits by project and district mentors. The topics for improving childbirth were deliveries and partographs. Findings Median monthly deliveries increased in 4 months from 38 (IQR 37-40) to 65 (IQR 53-71) in Mtwara Rural district, and in 17 months in Ruangwa district from 110 (IQR 103-125) to 161 (IQR 148-174). In Ruangwa health facilities, the women for whom partographs were used to monitor labour progress increased from 10 to 57 per cent in 17 months. Research limitations/implications The time for QI innovation, testing and implementation phases was limited, and the study only looked at trends. The outcomes were limited to process rather than health outcome measures. Originality/value Healthcare providers became confident in the QI method through engagement, generating and testing their own change ideas, and observing improvements. The findings suggest that implementing a QI initiative is feasible in rural, low-income settings.
Quality and rural-urban comparison of tuberculosis care in Rivers State, Nigeria.

Nigeria ranks among countries with the highest burden of tuberculosis. Yet evidence continues to indicate poor treatment outcomes which have been attributed to poor quality of care. This study aims to identify some of the systemic problems in order to inform policy decisions for improved quality of services and treatment outcomes in Nigeria. A comparative assessment of the quality of TB care in rural and urban health facilities was carried out between May and June 2013, employing the Donabedian model of quality assessment. Data was analysed using the SPSS software package version 20.0. The level of significance was set at p < 0.05. Health facility infrastructures were more constrained in the urban than rural settings. Both the urban and rural facilities lacked adequate facilities for infection control such as, running water, air filter respirators, hand gloves and extractor fans. Health education and HIV counselling and testing (HCT) were limited in rural facilities compared to urban facilities. Although anti-TB drugs were generally available in both settings, the DOTS strategy in patient care was completely ignored. Finally, laboratory support for diagnosis and patient monitoring was limited in the rural facilities. The study highlights suboptimal quality of TB care in Rivers State with limitations in health education and HCT of patients for HIV as well as laboratory support for TB care in rural health facilities. We, therefore, recommend that adequate infection control measures, strict observance of the DOTS strategy and sufficient laboratory support be provided to TB clinics in the State.

Abortion is a common medical procedure, yet its availability has become more limited across the United States over the past decade. Women who do not know where to go for abortion care may use the internet to find abortion facility information, and there appears to be more online searches for abortion in states with more restrictive abortion laws. While previous studies have examined the distances women must travel to reach an abortion provider, to our knowledge no studies have used a systematic online search to document the geographic locations and services of abortion facilities. The objective of our study was to describe abortion facilities and services available in the United States from the perspective of a potential patient searching online and to identify US cities where people must travel the farthest to obtain abortion care. In early 2017, we conducted a systematic online search for abortion facilities in every state and the largest cities in each state. We recorded facility locations, types of abortion services available, and facility gestational limits. We then summarized the frequencies by region and state. If the online information was incomplete or unclear, we called the facility using a mystery shopper method, which simulates the perspective of patients calling for services. We also calculated distance to the closest abortion facility from all US cities with populations of 50,000 or more. We identified 780 facilities through our online search, with the fewest in the Midwest and South. Over 30% (236/780, 30.3%) of all facilities advertised the provision of medication abortion services only; this proportion was close to 40% in the Northeast (89/233, 38.2%) and West (104/262, 39.7%). The lowest gestational limit at which services were provided was 12 weeks in Wyoming; the highest was 28 weeks in New Mexico. People in 27 US cities must travel over 100 miles (160 km) to reach an abortion facility; the state with the largest number of such cities is Texas (n=10). Online searches can provide detailed information about the location of abortion facilities and the types of services they provide. However, these facilities are not evenly distributed geographically, and many large US cities do not have an abortion facility. Tanning facility use: are we exceeding Food and Drug Administration limits?

The US Food and Drug Administration (FDA) recommends exposure limits for tanning bed use. Tanning patrons may not be following these recommendations and may be overexposed to damaging ultraviolet radiation (UV). This study was conducted to assess tanning patrons' adherence to FDA-recommended exposure limits and to measure the amount of UVA and UVB radiation emitted by tanning beds. A community-based survey was administered during routine state inspections of North Carolina tanning facilities (n = 50). At each facility, patron records were randomly selected (n = 483) for a survey of exposure records, and UVA and UVB outputs were measured for each tanning bed. The recommended limits were exceeded by 95% of patrons, and 33% of patrons began tanning at the maximum doses recommended for maintenance tanning. Average tanning bed output was 192.1 W/m(2) UVA and 0.35 W/m(2) erythemally weighted UVB. Interventions for tanning bed operators and patrons are needed to increase compliance with federally recommended exposure limits.

Quality and rural-urban comparison of tuberculosis care in Rivers State, Nigeria

Introduction Nigeria ranks among countries with the highest burden of tuberculosis. Yet evidence continues to indicate poor treatment outcomes which have been attributed to poor quality of care. This study aims to identify some of the systemic problems in order to inform policy decisions for improved quality of services and treatment outcomes in Nigeria. Methods A comparative assessment of the quality of TB care in rural and urban health facilities was carried out between May and June 2013, employing the Donabedian model of quality assessment. Data was analysed using the SPSS software package version 20.0. The level of significance was set at p < 0.05. Results Health facility infrastructures were more constrained in the urban than rural settings. Both the urban and rural facilities lacked adequate facilities for infection control such as, running water, air filter respirators, hand gloves and extractor fans. Health education and HIV counselling and testing (HCT) were limited in rural facilities compared to urban facilities. Although anti-TB drugs were generally available in both settings, the DOTS strategy in patient care was completely ignored. Finally, laboratory support for diagnosis and patient monitoring was limited in the rural facilities. Conclusion The study highlights suboptimal quality of TB care in Rivers State with limitations in health education and HCT of patients for HIV as well as laboratory support for TB care in rural health facilities. We, therefore, recommend that adequate infection control measures, strict observance of the DOTS strategy and sufficient laboratory support be provided to TB clinics in the State. PMID:27642401

The cost and feasibility of retrofitting the National Orange Show Facilities to use geothermal heat instead of natural gas for heating are determined. Because of the limited usage of the smaller facilities the study was limited to the conversion of the six major buildings: Domed, Hobby, Citrus, Auditorium, Restaurant and Commercial. A major problem is that most of the buildings are used on a very limited basis. This drastically reduced the amount of savings that could be used to amortize the retrofit cost. Another problem is that the buildings are spread over a large area and so the below grademore » piping costs were high. Finally, all of the buildings except for the Auditorium have direct gas fired heaters that would require all new terminal heating systems. In order to limit the retrofit cost, the retrofit system was designed to handle less than the peak load. This seemed appropriate because the facility might not even be in operation when a peak load condition occurred. Also, the existing systems could be used to supplement the geothermal system if necessary. The calculated and design peak loads are summarized.« less

The southwest region of the United States is expected to experience an expansion of commercial solar photovoltaic generation facilities over the next 25 years. A solar facility converts direct current generated by the solar panels to three-phase 60-Hz power that is fed to the grid. This conversion involves sequential processing of the direct current through an inverter that produces low-voltage three-phase power, which is stepped up to distribution voltage (∼12 kV) through a transformer. This study characterized magnetic and electric fields between the frequencies of 0 Hz and 3 GHz at two facilities operated by the Southern California Edison Company in Porterville, CA and San Bernardino, CA. Static magnetic fields were very small compared to exposure limits established by IEEE and ICNIRP. The highest 60-Hz magnetic fields were measured adjacent to transformers and inverters, and radiofrequency fields from 5-100 kHz were associated with the inverters. The fields measured complied in every case with IEEE controlled and ICNIRP occupational exposure limits. In all cases, electric fields were negligible compared to IEEE and ICNIRP limits across the spectrum measured and when compared to the FCC limits (≥0.3 MHz).
An investigation of ADA compliance of aquatic facilities in the North Texas area.

The study expands research on accessibility, comparing compliance scores of aquatic facilities in North Texas built before the 1991 Title III Americans with Disabilities Act Accessibility Guidelines (ADAAG) with facilities built after the 1991 ADAAG and the proposed 2002 supplement. A quasi-experimental design directed the selection of 52 facilities where measurements were taken to determine compliance with ADAAG and the supplement. A focus group provided insight into interpreting which features functioned as barriers or constraints to participation. Metropolitan statistical area in North Texas. A total of 52 aquatic facilities and 12 focus group participants (University of North Texas institutional review board 07-283). ADA aquatic facility compliance instrument. Frequency, ratios. No facilities were 100% ADA compliant overall, although some facilities were 100% compliant with specific structural domains. Women's restrooms rated lowest (average = 55%), and men's restrooms received the second lowest rating (average = 64%). Focus group results indicated that improperly designed restrooms and pool entries are primary barriers to participation. The findings support a need for stronger enforcement of policies that improve accessibility of facilities. Architectural reviews and construction practices need to be improved. The structural barriers and constraints identified can be limiting factors in efforts aimed at increasing physical activity among individuals with disabilities and individuals with physical limitations.

Whether timeliness of follow-up after abnormal mammography differs at facilities serving vulnerable populations, such as women with limited education or income, in rural areas, and racial/ethnic minorities is unknown. We examined receipt of diagnostic evaluation after abnormal mammography using 1998-2006 Breast Cancer Surveillance Consortium-linked Medicare claims. We compared whether time to recommended breast imaging or biopsy depended on whether women attended facilities serving vulnerable populations. We characterized a facility by the proportion of mammograms performed on women with limited education or income, in rural areas, or racial/ethnic minorities. We analyzed 30,874 abnormal screening examinations recommended for follow-up imaging across 142 facilities and 10,049 abnormal diagnostic examinations recommended for biopsy across 114 facilities. Women at facilities serving populations with less education or more racial/ethnic minorities had lower rates of follow-up imaging (4%-5% difference, P<0.05), and women at facilities serving more rural and low-income populations had lower rates of biopsy (4%-5% difference, P<0.05). Women undergoing biopsy at facilities serving vulnerable populations had longer times until biopsy than those at facilities serving nonvulnerable populations (21.6 vs. 15.6 d; 95% confidence interval for mean difference 4.1-7.7). The proportion of women receiving recommended imaging within 11 months and biopsy within 3 months varied across facilities (interquartile range, 85.5%-96.5% for imaging and 79.4%-87.3% for biopsy). Among Medicare recipients, follow-up rates were slightly lower at facilities serving vulnerable populations, and among those women who returned for diagnostic evaluation, time to follow-up was slightly longer at facilities that served vulnerable population. Interventions should target variability in follow-up rates across facilities, and evaluate effectiveness particularly at facilities serving vulnerable populations.

An investigation has been conducted to determine how the choice of orbit for NASA's prospective Space IR Telescope Facility (SIRTF), between polar (99-deg) and low inclination (28.5-deg) alternatives, will affect the performance of the all-superfluid He-cooled IR optics employed. While the dewar design met both the service life and 200-micron background-limited performance criteria in the case of the polar orbit mission, the alternative orbit allowed the background-limited criteria to be met only 50 percent of the time. It is accordingly recommended that the 200-micron background-limited observations be made only for a limited portion of the mission, while meeting the 100-micron limit at all times.
