Rural health prepayment schemes in China: towards a more active role for government.

A large majority of China's rural population were members of health prepayment schemes in the 1970's. Most of these schemes collapsed during the transition to a market economy. Some localities subsequently reestablished schemes. In early 1997 a new government policy identified health prepayment as a major potential source of rural health finance. This paper draws on the experience of existing schemes to explore how government can support implementation of this policy. The decision to support the establishment of health prepayment schemes is part of the government's effort to establish new sources of finance for social services. It believes that individuals are more likely to accept voluntary contributions to a prepayment scheme than tax increases. The voluntary nature of the contributions limits the possibilities for risk-sharing and redistribution between rich and poor. This underlines the need for the government to fund a substantial share of health expenditure out of general revenues, particularly in poor localities. The paper notes that many successful prepayment schemes depend on close supervision by local political leaders. It argues that the national programme will have to translate these measures into a regulatory system which defines the responsibilities of scheme management bodies and local governments. A number of prepayment schemes have collapsed because members did not feel they got value for money. Local health bureaux will have to cooperate with prepayment schemes to ensure that health facilities provide good quality services at a reasonable cost. Users' representatives can also monitor performance. The paper concludes that government needs to clarify the relationship between health prepayment schemes and other actors in rural localities in order to increase the chance that schemes will become a major source rural health finance.

Although fuel poverty is becoming increasingly researched, there is very limited information currently available on the experiences of and effects on children living in fuel poverty. This paper examines the consequences of using prepayment metering, a payment method typically used by low-income households, on households with children. We present new results from two postal survey datasets, the Electricity Prepayment Meter Users' Survey undertaken in late 2010 and the follow-up survey undertaken in 2011, which explore the outcomes of prepayment metering and living on low-incomes for households with children. Among prepayment consumers, households with children experience greater levels of hardship. Households with children were statistically significantly more likely to cut back on grocery spending, and indicated greater levels of financial difficulty than childless households. Although there were no differences between the groups for most indicators of poor thermal comfort levels, households with children were statistically significantly more likely to report seeing their breath condensing indoors on at least one occasion during the winter. Policies to address fuel poverty should include protections for prepayment meter consumers, and households with children using this payment method who are especially vulnerable.

Group premiums in micro health insurance experiences from Tanzania.

The main objective was to assess how group premiums can help poor people in the informal economy prepay for health care services. A comparative approach was adopted to study four groups of informal economy operators (cobblers, welders, carpenters, small scale market retailers) focusing on a method of prepayment which could help them access health care services. Two groups with a total of 714 operators were organized to prepay for health care services through a group premium, while the other two groups with a total of 702 operators were not organized to prepay through this approach. They prepaid through individual premium, each operator paying from his or her sources. Data on the four groups which lived in the same city was collected through a questionnaire and focus group discussions. Data collected was focused on health problems, health seeking behaviour and payment for health care services. Training of all the groups on prepaid health care financing based on individual based premium payment and group based premium payment was done. Groups were then free to choose which method to use in prepaying for health care. Prepayment through the two methods was then observed over a period of three years. Trends of membership attrition and retention were documented for both approaches. Data collected showed that the four groups were similar in many respects. These similarities included levels of education, housing, and social services such as water supplies, health problems, family size and health seeking behaviour. At the end of a period of three years 76% of the members from the two groups who chose group premium payment were still members of the prepayment health scheme and were receiving health care. For the two groups which opted for individual premium payment only 15% of their members were still receiving health care services at the end of three years. Group premium is a useful tool in improving accessibility to health care services in the poorer segments of the population especially the informal economy operators

The impact of fare policies and fare structure on the selection of equipment was investigated, fare collection systems are described, hardware and technology related problems are documented, and the requirements of a fare collection simulation model are outlined. Major findings include: (1) a wide variation in the fare collection systems and equipment, caused primarily by historical precedence; (2) the reliability of AFC equipment used at BART and WMATA discouraged other properties from considering use of similar equipment; (3) existing equipment may not meet the fare collection needs of properties in the near future; (4) the cost of fare collection operation and maintenance is high; and (5) the relatively small market in fare collection equipment discourages new product development by suppliers. Recommendations for fare collection R&D programs include development of new hardware to meet rail transit needs, study of impacts of alternate fare policies increased communication among policymakers, and consensus on fare policy issues.

This paper aims to optimize a desirable fare structure for the public transit service along a bus-subway corridor with the consideration of those factors related to equity in trip, including travel distance and comfort level. The travel distance factor is represented by the distance-based fare strategy, which is an existing differential strategy. The comfort level one is considered in the area-based fare strategy which is a new differential strategy defined in this paper. Both factors are referred to by the combined fare strategy which is composed of distance-based and area-based fare strategies. The flat fare strategy is applied to determine a reference level of social welfare and obtain the general passenger flow along transit lines, which is used to divide areas or zones along the corridor. This problem is formulated as a bi-level program, of which the upper level maximizes the social welfare and the lower level capturing traveler choice behavior is a variable-demand stochastic user equilibrium assignment model. A genetic algorithm is applied to solve the bi-level program while the method of successive averages is adopted to solve the lower-level model. A series of numerical experiments are carried out to illustrate the performance of the models and solution methods. Numerical results indicate that all three differential fare strategies play a better role in enhancing the social welfare than the flat fare strategy and that the fare structure under the combined fare strategy generates the highest social welfare and the largest resulting passenger demand, which implies that the more equity factors a differential fare strategy involves the more desirable fare structure the strategy has. This paper looks at both the prepayment risks of housing mortgage loan credit default swaps (LCDS) as well as the fuzziness and hesitation of investors as regards prepayments by borrowers. It further discusses the first default pricing of a basket of LCDS in a fuzzy environment by using stochastic analysis and triangular intuition-based fuzzy set theory. Through the 'fuzzification' of the sensitivity coefficient in the prepayment intensity, this paper describes the dynamic features of mortgage housing values using the One-factor copula function and concludes with a formula for 'fuzzy' pricing the first default of a basket of LCDS. Using analog simulation to analyze the sensitivity of hesitation, we derive a model that considers what the LCDS fair premium is in a fuzzy environment, including a pure random environment. In addition, the model also shows that a suitable pricing range will give investors more flexible choices and make the predictions of the model closer to real market values.

The Fluid Acquisition and Resupply Experiment (FARE) program, managed by the Marshall Space Flight Center Space Propulsion Branch with Martin Marietta Civil Space and Communications as the contractor, consisted of two flights designated FARE I and FARE II. FARE I flew in December 1992 on STS-53 with a screen channel liquid acquisition device (LAD) and FARE II flew in June 1993 on STS-57 with a vane-type LAD. Thus, the FARE I and II flights represent the two basic LAD categories usually considered for in-space fluid management. Although both LAD types have been used extensively, the usefulness of the on-orbit data has been constrained by the lack of experimentation beyond predicted performance limits, including both propellant fill and expulsion. Therefore, the FARE tests were designed to obtain data that would satisfy two primary objectives: (1) Demonstrate the performance of the two types of LADs, screen channel and vane, and (2) support the anchoring of analytical models. Both flights were considered highly successful in meeting these two primary objectives.

Clearly visible in the mid-deck FARE setup is one of two 12.5-inch spherical tanks made of transparent acrylic, one to supply and one to receive fluids. The purpose of FARE is to investigate the dynamics of fluid transfer in microgravity and develop methods for transferring vapor-free propellants and other liquids that must be replenished in long-term space systems like satellites, Extended-Duration Orbiters (EDO), and Space Station Freedom. Eight times over an eight-hour test period, the mission specialists conducted the FARE experiment. A sequence of manual valve operations caused pressurized air from the bottles to force fluids from the supply tank to the receiver tank and back again to the supply tank. Baffles in the receiver tank controlled fluid motion during transfer, a fine-mesh screen filtered vapor from the fluid, and the overboard vent removed vapor from the receiver tank as the liquid rose. FARE is managed by NASA's Marshall Space Flight Center (MSFC) in Alabama. The basic equipment was developed by Martin Marietta for the Storable Fluid Management Demonstration. Susan L. Driscoll is the principal investigator.

Health care financing provides the resources and economic incentives for operating health systems and is a key determinant of health system performance. Equitable financing is based on: financial protection, progressive financing and cross-subsidies. This paper describes Uganda's health care financing landscape and documents the key equity issues associated with the current financing mechanisms. We extensively reviewed government documents and relevant literature and conducted key informant interviews, with the aim of assessing whether Uganda's health care financing mechanisms exhibited the key principles of fair financing. Uganda's health sector remains significantly under-funded, mainly relying on private sources of financing, especially out-of-pocket spending. At 9.6 % of total government expenditure, public spending on health is far below the Abuja target of 15% that GoU committed to. Prepayments form a small proportion of funding for Uganda's health sector. There is limited cross-subsidisation and high fragmentation within and between health financing mechanisms, mainly due to high reliance on out-of-pocket payments and limited prepayment mechanisms. Without compulsory health insurance and low coverage of private health insurance, Uganda has limited pooling of resources, and hence minimal cross-subsidisation. Although tax revenue is equitable, the remaining financing mechanisms for Uganda are inequitable due to their regressive nature, their lack of financial protection and limited cross-subsidisation. Overall, Uganda's current health financing is inequitable and fragmented. The government should take explicit action to promote equitable health care financing by establishing pre-payment schemes, enhancing cross-subsidisation mechanisms and through appropriate integration of financing mechanisms.

The concept of introducing a no-frills airline to the wealthy Arab region presented its risks. This independent study sought to position the new airline in the marketplace. After three focus groups and 400 self-administered surveys, safety (#1) and price (#2) are low-fare carrier considerations whereas safety (#1), punctuality (#2) and price (#3) apply for full-fare airlines. Recommended ways for the no-frills carrier to reach the market include newspaper ads, travel agent sales, online bookings, and call centers. Additionally, respondents appeared to evaluate this low-fare carrier as if it is a full-service airline.

Featured in the mid-deck FARE setup is fluid activity in one of two 12.5-inch spherical tanks made of transparent acrylic. Pictured is the receiver tank. The other tank, out of frame below, is for supplying fluids. The purpose of FARE is to investigate the dynamics of fluid transfer in microgravity and develop methods for transferring vapor-free propellants and other liquids that must be replenished in long-term space systems like satellites, Extended-Duration Orbiters (EDO), and Space Station Freedom. Eight times over an eight-hour test period, the mission specialists conducted the FARE experiment. A sequence of manual valve operations caused pressurized air from the bottles to force fluids from the supply tank to the receiver tank and back again to the supply tank. Baffles in the receiver tank controlled fluid motion during transfer, a fine-mesh screen filtered vapor from the fluid, and the overboard vent removed vapor from the receiver tank as the liquid rose. FARE is managed by NASA's Marshall Space Flight Center (MSFC) in Alabama. The basic equipment was developed by Martin Marietta for the Storable Fluid Management Demonstration. Susan L. Driscoll is the principal investigator.

Chromosomal translocation (CT) is of enormous clinical interest because this disorder is associated with various major solid tumors and leukemia. A tumor-specific fusion gene event may occur when a translocation joins two separate genes. Currently, various CT databases provide information about fusion genes and their genomic elements. However, no database of the roles of fusion genes, in terms of essential functional and regulatory elements in oncogenesis, is available. FARE-CAFE is a unique combination of CTs, fusion proteins, protein domains, domain-domain interactions, protein-protein interactions, transcription factors and microRNAs, with subsequent experimental information, which cannot be found in any other CT database. Genomic DNA information including, for example, manually collected exact locations of the first and second break points, sequences and karyotypes of fusion genes are included. FARE-CAFE will substantially facilitate the cancer biologist's mission of elucidating the pathogenesis of various types of cancer. Simultaneous visualization of transonic buffet on a rocket faring model using unsteady PSP measurement and Schlieren method

Simultaneous visualization technique of the combination of the unsteady Pressure-Sensitive Paint and the Schlieren measurement was introduced. It was applied to a wind tunnel test of a rocket faring model at the JAXA 2mx2m transonic wind tunnel. Quantitative unsteady pressure field was acquired by the unsteady PSP measurement, which consisted of a high-speed camera, high-power laser diode, and so on. Qualitative flow structure was acquired by the Schlieren measurement using a high-speed camera and Xenon lamp with a blue optical filter. Simultaneous visualization was achieved 1.6 kfps frame rate and it gave the detailed structure of unsteady flow fields caused by the unsteady shock wave oscillation due to shock-wave/boundary-layer interaction around the juncture between cone and cylinder on the model. Simultaneous measurement results were merged into a movie including surface pressure distribution on the rocket faring and spatial structure of shock wave system concerning to transonic buffet. Constructed movie gave a timeseries and global information of transonic buffet flow field on the rocket faring model visually.

Structural realism versus deployment realism: A comparative evaluation.

In this paper I challenge and adjudicate between the two positions that have come to prominence in the scientific realism debate: deployment realism and structural realism. I discuss a set of cases from the history of celestial mechanics, including some of the most important successes in the history of science. To the surprise of the deployment realist, these are novel predictive successes toward which theoretical constituents that are now seen to be patently false were genuinely deployed. Exploring the implications for structural realism, I show that the need to accommodate these cases forces our notion of "structure" toward a dramatic depletion of logical content, threatening to render it explanatorily vacuous: the better structuralism fares against these historical examples, in terms of retention, the worse it fares in content and explanatory strength. I conclude by considering recent restrictions that serve to make "structure" more specific. I show however that these refinements will not suffice: the better structuralism fares in specificity and explanatory strength, the worse it fares against history. In light of these case studies, both deployment realism and structural realism are significantly threatened by the very historical challenge they were introduced to answer. Extending coverage to informal sector populations in Kenya: design preferences and implications for financing policy.

Universal health coverage (UHC) is important in terms of improving access to quality health care while protecting households from the risk of catastrophic health spending and impoverishment. However, progress to UHC has been hampered by the measures to increase mandatory prepaid funds especially in low- and middle-income countries where there are large populations in the informal sector. Important considerations in expanding coverage to the informal sector should include an exploration of the type of prepayment system that is acceptable to the informal sector and the features of such a design that would encourage prepayment for health care among this population group. The objective of the study was to document the views of informal sector workers regarding different prepayment mechanisms, and critically analyze key design features of a future health system and the policy implications of financing UHC in Kenya. This was part of larger study which involved a mixed-methods approach. The following tools were used to collect data from informal sector workers: focus group discussions [N = 16 (rural = 7; urban = 9)], individual in-depth interviews [N = 26 (rural = 14; urban = 12)] and a questionnaire survey [N = 455(rural = 129; urban = 326)]. Thematic approach was used to analyze qualitative data while Stata v.11 involving mainly descriptive analysis was used in quantitative data. The tools mentioned were used to collect data to meet various objectives of a larger study and what is presented here constitutes a small section of the data generated by these tools. The findings show that informal sector workers in rural and urban areas prefer different prepayment systems for financing UHC. Preference for a non-contributory system of financing UHC was particularly strong in the urban study site (58%). Over 70% in the rural area preferred a contributory mechanism in financing UHC. The main concern for informal sector workers regardless of the overall design of the financing approach to UHC included a poor governance culture especially one that does not punish corruption. Other reasons especially with regard to the contributory financing approach included high premium costs and inability to enforce contributions from informal sector. On average 47% of all study participants, the largest single majority, are in favor of a non-contributory financing mechanism. Strong evidence from existing literature indicates difficulties in implementing social contributions as the primary financing mechanism for UHC in contexts with large informal sector populations. Non-contributory financing should be strongly recommended to policymakers to be the primary financing mechanism and supplemented by social contributions.

Essays on pricing dynamics, price dispersion, and nested logit modelling

The body of this dissertation comprises three standalone essays, presented in three respective chapters. Chapter One explores the possibility that local market power contributes to the asymmetric relationship observed between wholesale costs and retail prices in gasoline markets. I exploit an original data set of weekly gas station prices in Southern California from September 2002 to May 2003, and take advantage of highly detailed station and local market-level characteristics to determine the extent to which spatial differentiation influences price-response asymmetry. I find that brand identity, proximity to rival stations, bundling and advertising, operation type, and local market features and demographics each influence a station's predicted asymmetric relationship between prices and wholesale costs. Chapter Two extends the existing literature on the effect of market structure on price dispersion in airline fares by modeling the effect at the disaggregate ticket level. Whereas past studies rely on aggregate measures of price dispersion such as the Gini coefficient or the standard deviation of fares, this paper estimates the entire empirical distribution of airline fares and documents how the shape of the distribution is determined by market structure. Specifically, I find that monopoly markets favor a wider distribution of fares with more mass in the tails while duopoly and competitive markets exhibit a tighter fare distribution. These findings indicate that the dispersion of airline fares may result from the efforts of airlines to practice second-degree price discrimination. Chapter Three adopts a Bayesian approach to the problem of tree structure specification in nested logit modelling, which requires a heavy computational burden in calculating marginal likelihoods. I compare two different techniques for estimating marginal likelihoods: (1) the Laplace approximation, and (2) reversible jump MCMC. I apply the techniques to both a simulated and a travel mode choice data set, and find that model selection is invariant to prior specification, while model derivatives like willingness-to-pay are notably sensitive to model choice. I also find that the Laplace approximation is remarkably accurate in spite of the potential for nested logit models to have irregular likelihood surfaces.

U.S. District Judge Joe Billy McDade ruled that the Metro Link bus system of Rock Island, IL cannot exclude people with AIDS from a special rate program afforded to those with mobility, visual, or hearing impairments. The suit was brought by the American Civil Liberties Union on behalf of [name removed] who wished to obtain a reduced-fare card because his AIDS condition severely impaired his ability to walk. Metro Link's application for reduced fare explicitly states that those whose sole disability is AIDS are ineligible for the program. Judge McDade ruled that the application violates the American Disabilities Act (ADA) and the Rehabilitation Act because it discriminated against [name removed] exclusively because of his disability. Airline Choice for Domestic Flights in Sao Paulo Metropolitan Area: An Application of the Conditional Logit Model

Using the conditional (multinomial) LOGIT model, this paper addresses airline choice in the S o Paulo Metropolitan Area. There are two airports in this region, where two, three or even four airlines compete for passengers flying to an array of domestic destinations. The airline choice is believed to be a result of the tradeoff passengers face among flight cost, flight frequency and airline performance. It was found that the lowest fare better explains airline choice than the highest fare, whereas direct flight frequencies give better explanation to airline choice than indirect (connections and stops) and total (direct plus indirect) ones. Out of 15 variables tested, the lowest fare was the variable that best explained airline choice. However, its signal was counterintuitive (positive) possibly because the cheapest airline was offering few flights, so passengers overwhelmingly failed to choose the cheapest airline. The model specification most adjusted to the data considered the lowest fare, direct flight frequency in the travel day and period (morning or afternoon peak) and airline age. Passengers departing from S o Paulo-Guarulhos International Airport (GRU) airport make their airline choice in terms of cost whereas those from Sao Paulo-Congonhas Airport (CGH) airport do not. Finally, senior passengers place more importance on airline age than junior passengers.

The Fluid Acquisition and Resupply Experiment, (FARE) is a Shuttle middeck-mounted experiment to demonstrate techniques for handling liquids in zero gravity for operations such as refueling spacecraft in orbit. The first flight took place on STS 53 launched December 2, 1992. Eight tests were performed during the mission and the experiment achieved 100 percent mission success. The second flight will be on STS 57, scheduled for launch in June 1993. The objective of FARE I was to demonstrate techniques for controlling the position of the liquid and gas within a tank during refilling and to better understand the operation of screen-type surface tension devices used to drain tanks in zero gravity. Tests were performed to demonstrate tank refilling, low gravity propellant slosh, and expulsion efficiency of the screen device. Expulsion efficiencies of 97 percent - 98 percent were demonstrated under a variety of flowrates and accelerations. Final fill levels of 60 percent to 80 percent were achieved during the vented fill tests.

A modal shift occurs when one mode of transport has a comparative advantage in a similar market over another. The present work concerns with the development of modal shift model for urban travel in Chennai, India. The modal shift model was calibrated using binary logit technique and validated using hold-out sample method. The validated model was used to predict the probability of shift in selected corridor. The recent introduction of metro rail in Chennai has lead to an increasing competition among public transport modes. To study the influence of metro on bus transport, a Stated Preference (SP) survey was conducted among express bus travellers. Using the SP survey data, a modal shift model was calibrated to estimate the plausible shift from bus to metro. Results indicate that variables like fare-difference, age, and income play an important role in the shift behaviour. When metro fare increases with respect to express bus fare, bus passengers are less willing to use metro and vice-versa.
