This Handbook sets forth information for the guidance of all users of the NASA Forms Management Program System. It is issued in accordance with the Federal Information Resources Management Regulation (FIRMR), Subpart 201-9.1. This Handbook sets forth an alpha-functional index of NASA-prefixed forms by title, identifying number, and unit of issue. The automated processing two-letter code (NF) has been substituted for the spelling out of the NASA form-prefix preceding the form number. To indicate a description in lieu of a distinct title, the entire reference under the Form Title/Description column has been enclosed in parentheses. A list of current forms, shown by number and page, is included for cross-reference and to preclude the ordering of those forms which have been deleted from the system. This Handbook will be updated, as appropriate. NHB 1420.2H dated July 1986, is cancelled.

A network of interconnected processors is formed from a vertex symmetric graph selected from graphs .GAMMA..sub.d (k) with degree d, diameter k, and (d+1)!/(d-k+1)! processors for each d.gtoreq.k and .GAMMA..sub.d (k,-1) with degree 3-1, diameter k+1, and (d+1)!/(d-k+1)! processors for each d.gtoreq.k.gtoreq.4. Each processor has an address formed by one of the permutations from a predetermined sequence of letters chosen a selected number of letters at a time, and an extended address formed by appending to the address the remaining ones of the predetermined sequence of letters. A plurality of transmission channels is provided from each of the processors, where each processor has one less channel than the selected number of letters forming the sequence. Where a network .GAMMA..sub.d (k,-1) is provided, no processor has a channel connected to form an edge in a direction .delta..sub.1. Each of the channels has an identification number selected from the sequence of letters and connected from a first processor having a first extended address to a second processor having a second address formed from a second extended address defined by moving to the front of the first extended address the letter found in the position within the first extended address defined by the channel identification number. Leprosy is a chronic granulomatous infection that manifests as different clinical forms related to the immunological response. The aim of the study was to evaluated the response of IL-22, STAT3, CD68 and iNOS in leprosy skin lesions. The mean number IL-22 positive cells was 12.12±1.90cells/field in the TT form and 31.31±2.91cells/field in the LL form. STAT3 positive cells was 5.29±1.96 cells/field in the TT form, while this number was 11.13±3.48cells/field in the LL form. The mean number of CD68 positive cells was 25.18±6.21cells/field in the TT form and 62.81±8.13cells/field in the LL form. Quantitative analysis of iNOS revealed a significant difference, with the mean number of cells expressing the enzyme being 30.24±2.88cells/field in the TT form compared to 35.44±4.69cells/field in the LL form. Linear correlations in lesions of TT patients showed a moderate positive correlations between CD68 and iNOS, STAT3 and Inos, IL-22 and STAT3, and IL-22 and iNOS. Our results demonstrate that these factors can act synergistically to induce a microbicidal activity in the population of macrophages in the leprosy lesions. Copyright © 2017 Elsevier B.V. Evaluation of request forms submitted to the haematology laboratory in a Ghanaian tertiary hospital.

Laboratory request forms provide information about the laboratory test being requested for. They carry demographic data and other information such as location of patient, laboratory number, doctor's name, signature of the doctor, telephone number of the requesting doctor. Omission of information on the forms may lead to laboratory errors. The aim of this study was to evaluate the level of completion of laboratory request forms at the haematology department of a Ghanaian tertiary hospital. Three thousand request forms submitted to the haematology department between January and April 2010 were retrieved and studied. The information provided on each request form was recorded in a spread sheet and analyzed. The patient's age and sex were missing in 25.6% and 32.7% of the forms respectively. About half of the request forms did not have the patient's location. No clinical detail was provided on 22.7% of the forms. Doctors were more likely to sign their request forms and provide a name but they all failed to provide an address or a contact telephone number. This study demonstrates that, the standard of completion of request forms was poor. Essential information required on the forms was often missing. This can lead to limited advice given by laboratory physicians and may increase the potential for errors. Conversely, provision of all the information needed on the forms will aid laboratory diagnosis and enhance patient care and save time and resources. There should be closer interaction between clinicians and laboratory personnel to improve quality of services.

Graviceptor development in jellyfish ephyrae in space and on Earth

Graviceptor (rhopalium) development in Aurelia aurita ephyrae which developed on Earth and in space during the nine-day NASA SLS-1 mission was compared. The space-developed ephyrae made graviceptors which were morphologically similar to those of their ground-based controls. Rhopalia of both groups developed statocysts with statoliths, ocelli, ciliated mechanoreceptor cells, and immature touch-plates with one type of hair cell. The number of rhopalia formed per arm of ephyrae of both groups revealed no significant differences. The number of statoliths formed per rhopalium was statistically higher in ephyrae which were induced to form in space with iodine than in L(Launch)+8h controls. Statolith numbers were not significantly different between Earth-formed control ephyrae and those formed from polyps induced on Earth and then sent into space 24h and 48h later. Statolith loss from rhopalia was significantly enhanced in the space-maintained ephyrae in artificial sea water (ASW) as compared to their controls. Ephyrae formed through thyroxine treatment and those maintained in thyroxine in space had statolith numbers comparable to thyroxine-treated controls. Pulsing abnormalitities seen in some space-developed ephyrae suggest that some space-formed ephyrae may have developed abnormal rhopalia because normal rhopalia development and function is necessary for normal pulsing.

Collection of feedback regarding medical student clinical experiences for formative or summative purposes remains a challenge across clinical settings. The purpose of this study was to determine whether the use of a quick response (QR) code-linked online feedback form improves the frequency and efficiency of rater feedback. In 2016, we compared paper-based feedback forms, an online feedback form, and a QR code-linked online feedback form at 15 family medicine clerkship sites across the United States. Outcome measures included usability, number of feedback submissions per student, number of unique raters providing feedback, and timeliness of feedback provided to the clerkship director. The feedback method was significantly associated with usability, with QR code scoring the highest, and paper second. Accessing feedback via QR code was associated with the shortest time to prepare feedback. Across four rotations, separate repeated measures analyses of variance showed no effect of feedback system on the number of submissions per student or the number of unique raters. The results of this study demonstrate that preceptors in the family medicine clerkship rate QR code-linked feedback as a high usability platform. Additionally, this platform resulted in faster form completion than paper or online forms. An overarching finding of this study is that feedback forms must be portable and easily accessible. Potential implementation barriers and the social norm for providing feedback in this manner need to be considered.

The form of physical property tensors of a quasi-one-dimensional material such as a nanotube or a polymer can be determined from the point group of its symmetry group, one of an infinite number of line groups. Such forms are calculated using a method based on the use of trigonometric summations. With this method, it is shown that materials invariant under infinite subsets of line groups have physical property tensors of the same form. For line group types of a family of line groups characterized by an index n and a physical property tensor of rank m, the form of the tensor for all line group types indexed with n > m is the same, leaving only a finite number of tensor forms to be determined.

Does Grammatical Number Influence the Semantic Priming Between Number Cues and Words Related to Vertical Space? An Investigation Using Virtual Reality.

The GES framework postulates a hierarchical order between grounded, embodied, and situated representations. Against this background, the present study investigated the relation of two effects: (i) a semantic priming between number cues and words with referents up or down in the world according to the number's magnitude which is supposed to be grounded (cf. Lachmair et al., 2014) and (ii) the compatibility between number cues and the grammatical word form of the words according to the number's multitude which is supposed to be embodied (cf. Roettger and Domahs, 2015). In two experiments words referring to objects up or down in the world and spatially neutral words were presented subsequent to the numbers "1" and "9." In Experiment 1 words were presented in singular word form and in Experiment 2 in plural word form. For the first time, Virtual Reality was used in such an experimental setup in order to reduce spatial predispositions of participants and to provide a homogeneous experimental environment for replication purposes. According to GES it was expected that the spatial semantic priming should occur in both grammatical word forms. However, the compatibility with grammatical number should only occur for the plural word form due to its markedness. The results of Experiment 1 support the spatial-semantic-priming-hypothesis but not the grammatical-number-hypothesis. The results of Experiment 2 supported only the grammatical-number-hypothesis. It is argued that the grounded spatial effect of Experiment 1 was not affected by grammatical number. However, in Experiment 2 this effect vanished due to an activated embodied reference frame according to grammatical number.

To determine effects of storage temperature and time on pH and specific gravity of and number and size of crystals in urine samples from dogs and cats. Randomized complete block design. 31 dogs and 8 cats. Aliquots of each urine sample were analyzed within 60 minutes of collection or after storage at room or refrigeration temperatures (20 vs 6 degrees C [68 vs 43 degrees F]) for 6 or 24 hours. Crystals formed in samples from 11 of 39 (28%) animals. Calcium oxalate (CaOx) crystals formed in vitro in samples from 1 cat and 8 dogs. Magnesium ammonium phosphate (MAP) crystals formed in vitro in samples from 2 dogs. Compared with aliquots stored at room temperature, refrigeration increased the number and size of crystals that formed in vitro; however, the increase in number and size of MAP crystals in stored urine samples was not significant. Increased storage time and decreased storage temperature were associated with a significant increase in number of CaOx crystals formed. Greater numbers of crystals formed in urine aliquots stored for 24 hours than in aliquots stored for 6 hours. Storage time and temperature did not have a significant effect on pH or specific gravity. Urine samples should be analyzed within 60 minutes of collection to minimize temperature- and time-dependent effects on in vitro crystal formation. Presence of crystals observed in stored samples should be validated by reevaluation of fresh urine.

Based on the negabinary number representation, parallel one-step arithmetic operations (that is, addition and subtraction), logical operations, and matrix-vector multiplication on data have been optically implemented, by use of a two-dimensional spatial-encoding technique. For addition and subtraction, one of the operands in decimal form is converted into the unsigned negabinary form, whereas the other decimal number is represented in the signed negabinary form. The result of operation is obtained in the mixed negabinary form and is converted back into decimal. Matrix-vector multiplication for unsigned negabinary numbers is achieved through the convolution technique. Both of the operands for logical operation are converted to their signed negabinary forms. All operations are implemented by use of a unique optical architecture. The use of a single liquid-crystal-display panel to spatially encode the input data, operational kernels, and decoding masks have simplified the architecture as well as reduced the cost and complexity.

The genital malformations in Mayer-Rokitansky-Küster-Hauser syndrome (MRKH) are frequently accompanied by associated malformations whose forms were recently classified as typical (isolated uterovaginal aplasia/hypoplasia) and atypical (the addition of malformations in the ovary or renal system). The aim of this study was to compare the surrogate IVF performance of women with typical and atypical forms including their chances of achieving pregnancy. The follow-up data on a total of 102 cycles of surrogate IVF in 27 MRKH patients treated in our department between 2000 and 2010 were analysed. Twenty patients with the typical form who underwent 72 IVF cycles were compared with seven patients with the atypical form who underwent 30 IVF cycles. The various examined parameters of these intended mothers were age, hormonal profile during controlled ovarian hyperstimulation and laboratory outcome. The mean number of gonadotrophin ampoules needed for stimulation and treatment duration was significantly higher in the atypical form (3600 ± 1297IU for 13 ± 2.3 days versus 2975 ± 967 IU for 11.6 ± 1.6 days, P≤ 0.01). Serum estradiol and progesterone levels measured on the hCG administration day were similar. A significantly higher mean number of follicles 12.6 ± 6 versus 8.9 ± 5.4, P≤ 0.03, metaphase II (MII) oocytes 8.7 ± 5.1 versus 6.7 ± 4.8, P≤ 0.05, fertilizations 6 ± 3.6 versus 4.4 ± 3.3, P≤ 0.03 and cleaving embryos 5.7 ± 3.8 versus 4.1 ± 3.3, P≤ 0.01 were available in patients with the typical form compared with those with the atypical form, respectively. There was no significant difference in fertilization rate, cleavage rate or the mean number of transferred embryos. Embryo quality of the transferred ones and pregnancy rate per cycle were also similar between the two groups. Women with the typical form of MRKH needed fewer gonadotrophins and for a shorter duration for ovarian hyperstimulation. The mean number of follicles, oocytes, MII oocytes, fertilizations and cleaving embryos was higher among women with the typical form. Pregnancy rates were similar since the available number and quality of transferred embryos to the surrogate mother were not affected.

Gene regulatory circuits are to the cell what arithmetic logic units are to the chip: fundamental components of information processing that map an input onto an output. Gene regulatory circuits come in many different forms, distinct structural configurations that determine who regulates whom. Studies that have focused on the gene expression patterns (functions) of circuits with a given structure (form) have examined just a few structures or gene expression patterns. Here, we use a computational model to exhaustively characterize the gene expression patterns of nearly 17 million three-gene circuits in order to systematically explore the relationship between circuit form and function. Three main conclusions emerge. First, function does not follow form. A circuit of any one structure can have between twelve and nearly thirty thousand distinct gene expression patterns. Second, and conversely, form does not follow function. Most gene expression patterns can be realized by more than one circuit structure. And third, multifunctionality severely constrains circuit form. The number of circuit structures able to drive multiple gene expression patterns decreases rapidly with the number of these patterns. These results indicate that it is generally not possible to infer circuit function from circuit form, or vice versa.

FormTracer. A mathematica tracing package using FORM

We present FormTracer, a high-performance, general purpose, easy-to-use Mathematica tracing package which uses FORM. It supports arbitrary space and spinor dimensions as well as an arbitrary number of simple compact Lie groups. While keeping the usability of the Mathematica interface, it relies on the efficiency of FORM. An additional performance gain is achieved by a decomposition algorithm that avoids redundant traces in the product tensors spaces. FormTracer supports a wide range of syntaxes which endows it with a high flexibility. Mathematica notebooks that automatically install the package and guide the user through performing standard traces in space-time, spinor and gauge-group spaces are provided. High-Pressure High-Temperature Phase Diagram of the Organic Crystal Paracetamol

High-pressure high-temperature (HPHT) Raman spectroscopy studies have been performed on the organic crystal paracetamol in a diamond anvil cell utilizing boron-doped diamond as heating anvil. The HPHT data obtained from boron-doped diamond heater is cross-checked with data obtained using a standard block heater diamond anvil cell. Isobaric measurements were conducted at pressures up to 8.5 GPa and temperature up to 520 K in a number of different experiments. Solid state phase transitions from monoclinic Form I --> orthorhombic Form II were observed at various pressures and temperatures as well as transitions from Form II --> unknown Form IV. The melting temperature for paracetamol was observed to increase with increasing pressures to 8.5 GPa. Our previous angle dispersive x-ray diffraction studies at the Advanced Photon Source has confirmed the existence of two unknown crystal structures Form IV and Form V of paracetamol at high pressure and ambient temperature. The phase transformation from Form II to Form IV occurs at ~8.5 GPa and from Form IV to Form V occurs at ~11 GPa at ambient temperature. Our new data is combined with the previous ambient temperature high-pressure Raman and X- ray diffraction data to create the first HPHT phase diagram of paracetamol. Dissenting from care.data: an analysis of opt-out forms.

Care.data was a programme of work led by NHS England for the extraction of patient-identifiable and coded information from general practitioner (GP) records for secondary uses. This study analyses the forms (on the websites of GP practices) which enabled patients to opt out. Theoretical sampling and summative content analysis were used to collect and analyse dissent forms used by patients to opt out from care. Domains included basic information about the programme, types of objections and personal details required for identification purposes. One hundred opt-out forms were analysed. Fifty-four forms mentioned that this programme was run by NHS England. 81 forms provided two types of objections to data-sharing, and 15 provided only one objection. Only 26 forms mentioned that direct care would not be affected and 32 that patients maintain their right to opt back anytime. All but one of the opt-out forms we reviewed requested the name of the person wishing to opt out. 94 required a date of birth and 33 an NHS number. 82 required an address, 42 a telephone number and 7 an email address. Numbers of patients (not) opting out should be treated with caution, because the variability of information provided and the varied options for dissent may have caused confusion among patients. To ensure that dissent is in accordance with individual preferences and moral values, we recommend that well-designed information material and standardised opt-out forms be developed for such data-sharing initiatives. The effects of rotor downwash on helicopter fuselage aerodynamic characteristics were investigated. A rotor model for generating the downwash was mounted close to each of three fuselage models. The main report presents the force and moment data in both graphical and tabular form and the pressure data in graphical form. This supplement presents the pressure data in tabular form. Each run or parameter sweep is identified by a unique run number. The data points in each run are identified by a point number. The pressure data can be matched to the force data by matching the run and point number.

While the product of finitely many convex functions has been investigated in the field of global optimization, some fundamental issues such as the convexity condition and the Legendre-Fenchel transform for the product function remain unresolved. Focusing on quadratic forms, this paper is aimed at addressing the question: When is the product of finitely many positive definite quadratic forms convex, and what is the Legendre-Fenchel transform for it? First, we show that the convexity of the product is determined intrinsically by the condition number of so-called 'scaled matrices' associated with quadratic forms involved. The main result claims that if the conditionmore » number of these scaled matrices are bounded above by an explicit constant (which depends only on the number of quadratic forms involved), then the product function is convex. Second, we prove that the Legendre-Fenchel transform for the product of positive definite quadratic forms can be expressed, and the computation of the transform amounts to finding the solution to a system of equations (or equally, finding a Brouwer's fixed point of a mapping) with a special structure. Many of the differential equations arising in fluid dynamics may be stated in conservation-law form. A number of investigations have been conducted with the aim to derive the conservation-law form of the Navier-Stokes equations in general nonsteady coordinate systems. The present note has the objective to illustrate a mathematical methodology with which such forms of the equations may be derived in an easier and more general fashion. For numerical applications, the scalar form of the equations is eventually provided. Attention is given to the conservation form of equations in curvilinear coordinates and numerical considerations.

Workflow Enhancement (WE) Improves Safety in Radiation Oncology: Putting the WE and Team Together

Purpose: To review the impact of a workflow enhancement (WE) team in reducing treatment errors that reach patients within radiation oncology. Methods and Materials: It was determined that flaws in our workflow and processes resulted in errors reaching the patient. The process improvement team (PIT) was developed in 2010 to reduce errors and was later modified in 2012 into the current WE team. Workflow issues and solutions were discussed in PIT and WE team meetings. Due to tensions within PIT that resulted in employee dissatisfaction, there was a 6-month hiatus between the end of PIT and initiation of the renamed/redesigned WEmore » team. In addition to the PIT/WE team forms, the department had separate incident forms to document treatment errors reaching the patient. These incident forms are rapidly reviewed and monitored by our departmental and institutional quality and safety groups, reflecting how seriously these forms are treated. The number of these incident forms was compared before and after instituting the WE team. Results: When PIT was disbanded, a number of errors seemed to occur in succession, requiring reinstitution and redesign of this team, rebranded the WE team. Interestingly, the number of incident forms per patient visits did not change when comparing 6 months during the PIT, 6 months during the hiatus, and the first 6 months after instituting the WE team (P=.85). However, 6 to 12 months after instituting the WE team, the number of incident forms per patient visits decreased (P=.028). After the WE team, employee satisfaction and commitment to quality increased as demonstrated by Gallup surveys, suggesting a correlation to the WE team. Conclusions: A team focused on addressing workflow and improving processes can reduce the number of errors reaching the patient. Time is necessary before a reduction in errors reaching patients will be seen.« less

The wave number of characteristic absorption peak nu asC-O-C of the polyester formed on the frictional process were determined by Fourier Transform Infrared (FTIR) Micro-spectroscopy, and the wave number displacement of characteristic absorption peak nu asC-O-C was analyzed based on the conversion mass of polyester formed. The internal relations between anti-wear order rule of hydroxyl fatty acids and vibration absorption peak nu asC-O-C of polyester formed by hydroxyl fatty acids was deduced according to these results, and the anti-wear order of hydroxyl fatty acids was reasonably explained, that is 13, 14-di-hydroxydocosanoic acid > 13 (14)-monohydroxydocosanoic acid = 9,10-dihydroxyoctadecanoic acid > 9,10,12-trihydroxyoctadecanoic acid > 9(10)-monohydroxyoctadecanoic acid. A net polyester film is formed by 13, 14-dihydroxydocosanoic acid and a linear polyester film is formed by 9, (10)-monohydroxyoctadecanoic acid and 13(14)-monohydroxydocosanoic acid.

Myrtle is an evergreen shrub or small tree widespread throughout the Mediterranean region. In Turkey, both cultivated and wild forms, differing in plant and fruit size and fruit composition, can be found. These differences may have resulted from the domestication of the cultivated form over a long period of time. We investigated whether wild and cultivated forms of myrtle differ in karyological features (i.e., number of somatic chromosomes and relative genome size). We sampled two wild forms and six cultivated types of myrtle. All the samples had the same chromosome number (2n = 2x = 22). The results were confirmed by 4',6-diamidino-2-phenylindole (DAPI) flow cytometry. Only negligible variation (approximately 3%) in relative fluorescence intensity was observed among the different myrtle accessions, with wild genotypes having the smallest values. We concluded that despite considerable morphological differentiation, cultivated and wild myrtle genotypes in Turkey have similar karyological features.

Deep in the occipitotemporal cortex lie two functional regions, the visual word form area (VWFA) and the number form area (NFA), which are thought to play a special role in letter and number recognition, respectively. We review recent progress made in characterizing the origins of these symbol form areas in children or adults, sighted or blind subjects, and humans or monkeys. We propose two non-mutually-exclusive hypotheses on the origins of the VWFA and NFA: the presence of a connectivity bias, and a sensitivity to shape features. We assess the explanatory power of these hypotheses, describe their consequences, and offer several experimental tests. Copyright © 2015 Elsevier Ltd. All rights reserved.

The splash phenomenon is being increasingly explored with the use of modern measurement tools, including the high-speed cameras. Recording images at a rate of several thousand frames per second facilitates parameterization and description of the dynamics of splash phases. This paper describes the impact of a single drop of a liquid falling on the surface of the same liquid. Three single-phase liquid systems, i.e., water, petrol, and diesel fuel, were examined. The falling drops were characterized by different kinetic energy values depending on the height of the fall, which ranged from 0.1 to 7.0 m. Four forms, i.e., waves, crowns, semi-closed domes, and domes, were distinguished depending on the drop energy. The analysis of the recorded images facilitated determination of the static and dynamic parameters of each form, e.g., the maximum height of each splash form, the width of the splash form at its maximum height, and the rate of growth of the splash form. We, Re, Fr, and K numbers were determined for all analyzed liquid systems. On the basis of the obtained values of dimensionless numbers, the areas of occurrence of characteristic splash forms were separated.

This paper considered Rayleigh-Benard convection (natural convection). This is a flow, which is formed in a viscous medium when heated from below and cooled from above. As a result, are formed vortices (convective cells). This process is described by a system of nonlinear differential equations in Oberbeck-Boussinesq approximation. As the governing parameters characterizing convection states Rayleigh number, Prandtl number are picked. The problem is solved by using finite element method with computational package FEniCS. Numerical results for different Rayleigh numbers are obtained. Studied integral characteristic (Nusselt number) depending on the Rayleigh number.

The results of pressure-distribution and force tests of four wings at a Mach number of about 6.9 and a Reynolds number of 0.98 x 10(6) in the Langley 11-inch hypersonic tunnel are presented. The wings had a square plan form, a 5-percent-chord maximum thickness, and diamond, half-diamond, wedge, and half-circular sections.

Estimating Traffic Accidents in Turkey Using Differential Evolution Algorithm

Estimating traffic accidents play a vital role to apply road safety procedures. This study proposes Differential Evolution Algorithm (DEA) models to estimate the number of accidents in Turkey. In the model development, population (P) and the number of vehicles (N) are selected as model parameters. Three model forms, linear, exponential and semi-quadratic models, are developed using DEA with the data covering from 2000 to 2014. Developed models are statistically compared to select the best fit model. The results of the DE models show that the linear model form is suitable to estimate the number of accidents. The statistics of this form is better than other forms in terms of performance criteria which are the Mean Absolute Percentage Errors (MAPE) and the Root Mean Square Errors (RMSE). To investigate the performance of linear DE model for future estimations, a ten-year period from 2015 to 2024 is considered. The results obtained from future estimations reveal the suitability of DE method for road safety applications.

To design an excel form which can prompt dental implant patient appointment through color change, which can scientifically manage implant EMR library through appropriate interlinkage and number. An excel form based on operating system Windows XP was designed and software 2003 Microsoft excel was used, which was configured to change color with the passage of time by the use of command "conditional format". An excel form was designed. The color turned to red automatically on the day the patient underwent implant surgery. It turned to yellow when the patient recalled 2 weeks after the first operation, to green when the patient underwent secondary operation. It was designed to be gray when all the procedures of implant restoration was finished. In addition, we could know patients' main implant situation through directly opening his EMR when clicking on his name or number. Dentists can remind the implant patient appointment schedule through color change of an excel form, and can consult the implant patient EMR directly through interlinkage or number.
