Health services and policy research in the first decade at the Canadian Institutes of Health Research.

Health services and policy research is the innovation engine of a health care system. In 2000, the Canadian Institutes of Health Research (CIHR) was formed to foster the growth of all sciences that could improve health care. We evaluated trends in health services and policy research funding, in addition to determinants of funding success. All applications submitted to CIHR strategic and open operating grant competitions between 2001 and 2011 were included in our analysis. Age, sex, size of research team, critical mass, season, year and research discipline were retrieved from application information. A cohort of 4725 applicants successfully funded between 2001 and 2005 were followed for 5 years to evaluate predictors of continuous funding. Multivariate generalized estimating equation logistic regression was used to estimate predictors of funding success and sustained funding. Between 2001 and 2011, 80 163 applications were submitted to open and strategic grant competitions. Over time, grant applications increased from 327 to 1137 per year, and annual funding increased from $12.6 to $48.0 million. Grant applications from young male researchers were more likely to be funded than those from female researchers (odds ratio [OR] 1.40, 95% confidence interval [CI] 1.01-1.95), as were applications from larger research teams and institutions with a large critical mass. Only 24.0% of scientists whose first funded grant was in health services and policy research had sustained 5-year funding, compared with 52.8% of biomedical scientists (OR 0.34, 95% CI 0.24-0.49). The CIHR has successfully increased the amount of health services and policy research in Canada. To enhance conditions for success, researchers should be encouraged to work in teams, request longer duration grants, resubmit unsuccessful applications and affiliate themselves with institutions with a greater critical mass.

A review of studies fulfilled at the Lebedev Institute in collaboration with the Moscow State University and Institute of Atmospheric Optics in Tomsk on influence of various characteristics of ultrashort laser pulse on plasma channels formed under its filamentation is presented. Filamentation of high-power laser pulses with wavefront controlled by a deformable mirror, with cross-sections spatially formed by various diaphragms and with different wavelengths was experimentally and numerically studied. An application of plasma channels formed due to filamentation of ultrashort laser pulse including a train of such pulses for triggering and guiding long electric discharges is discussed. Low pressure polymer processes as thermoforming or rotational moulding use much simpler moulds than high pressure processes like injection. However, despite the low forces involved in the process, moulds manufacturing for these applications is still a very material, energy and time consuming operation. Particularly in rotational moulding there is no standard for the mould manufacture and very different techniques are applicable. The goal of this research is to develop and validate a method for manufacturing plastically formed sheet metal moulds by single point incremental forming (SPIF) for rotomoulding and rotocasting operations. A Stewart platform based SPIF machine allow the forming of thick metal sheets, granting the required structural stiffness for the mould surface, and keeping a short manufacture lead time and low thermal inertia. The experimental work involves the proposal of a hollow part, design and fabrication of a sheet metal mould using dieless incremental forming techniques and testing its operation in the production of prototype parts.

Indian pharmaceutical patent prosecution: The changing role of Section 3(d).

India, like many developing countries, only recently began to grant pharmaceutical product patents. Indian patent law includes a provision, Section 3(d), which tries to limit grant of "secondary" pharmaceutical patents, i.e. patents on new forms of existing molecules and drugs. Previous research suggests the provision was rarely used against secondary applications in the years immediately following its enactment, and where it was, was redundant to other aspects of the patent law, raising concerns that 3(d) was being under-utilized by the Indian Patent Office. This paper uses a novel data source, the patent office's first examination reports, to examine changes in the use of the provision. We find a sharp increase over time in the use of Section 3(d), including on the main claims of patent applications, though it continues to be used in conjunction with other types of objections to patentability. More surprisingly, see a sharp increase in the use of the provision against primary patent applications, contrary to its intent, raising concerns about potential over-utilization.

Cultural competency, trust, and research literacy can affect the planning and implementation of sustainable community-based participatory research (CBPR). The purpose of this manuscript is to highlight: (1) the development of a CBPR pilot grant request for application; and (2) a comprehensive program supporting CBPR obesity-related grant proposals facilitated by activities designed to promote scholarly collaborations between academic researchers and the community. After a competitive application process, academic researchers and non-academic community leaders were selected to participate in activities where the final culminating project was the submission of a collaborative obesity-related CBPR grant application. Teams were comprised of a mix of academic researchers and non-academic community leaders, and each team submitted an application addressing obesity-disparities among rural predominantly African American communities in the US Deep South. Among four collaborative teams, three (75%) successfully submitted a grant application to fund an intervention addressing rural and minority obesity disparities. Among the three submitted grant applications, one was successfully funded by an internal CBPR grant, and another was funded by an institutional seed funding grant. Preliminary findings suggest that the collaborative activities were successful in developing productive scholarly relationships between researchers and community leaders. Indian pharmaceutical patent prosecution: The changing role of Section 3(d)

India, like many developing countries, only recently began to grant pharmaceutical product patents. Indian patent law includes a provision, Section 3(d), which tries to limit grant of “secondary” pharmaceutical patents, i.e. patents on new forms of existing molecules and drugs. Previous research suggests the provision was rarely used against secondary applications in the years immediately following its enactment, and where it was, was redundant to other aspects of the patent law, raising concerns that 3(d) was being under-utilized by the Indian Patent Office. This paper uses a novel data source, the patent office’s first examination reports, to examine changes in the use of the provision. We find a sharp increase over time in the use of Section 3(d), including on the main claims of patent applications, though it continues to be used in conjunction with other types of objections to patentability. More surprisingly, see a sharp increase in the use of the provision against primary patent applications, contrary to its intent, raising concerns about potential over-utilization. PMID:29608604

This final report on computational methods and software systems for dynamics and control of large space structures covers progress to date, projected developments in the final months of the grant, and conclusions. Pertinent reports and papers that have not appeared in scientific journals (or have not yet appeared in final form) are enclosed. The grant has supported research in two key areas of crucial importance to the computer-based simulation of large space structure. The first area involves multibody dynamics (MBD) of flexible space structures, with applications directed to deployment, construction, and maneuvering. The second area deals with advanced software systems, with emphasis on parallel processing. The latest research thrust in the second area, as reported here, involves massively parallel computers.

In this study, a fuzzy query tool (SQLf) for non-fuzzy database management systems was developed. In addition, samples of fuzzy queries were made by using real data with the tool developed in this study. Performance of SQLf was tested with the data about the Marmara University students' food grant. The food grant data were collected in MySQL database by using a form which had been filled on the web. The students filled a form on the web to describe their social and economical conditions for the food grant request. This form consists of questions which have fuzzy and crisp answers. The main purpose of this fuzzy query is to determine the students who deserve the grant. The SQLf easily found the eligible students for the grant through predefined fuzzy values. The fuzzy query tool (SQLf) could be used easily with other database system like ORACLE and SQL server.

With the size reduction of nanoscale electronic devices, the heat generated by the unit area in integrated circuits will be increasing exponentially, and consequently the thermal management in these devices is a very important issue. In addition, the heat generated by the electronic devices mostly diffuses to the air in the form of waste heat, which makes the thermoelectric energy conversion also an important issue for nowadays. In recent years, the thermal transport properties in nanoscale systems have attracted increasing attention in both experiments and theoretical calculations. In this review, we will discuss various theoretical simulation methods for investigating thermal transport properties and take a glance at several interesting thermal transport phenomena in nanoscale systems. Our emphasizes will lie on the advantage and limitation of calculational method, and the application of nanoscale thermal transport and thermoelectric property. In this paper, the authors compare subgroup analyses as outlined in grant applications and their related publications. Grants awarded by the Netherlands Organization for Health Research and Development (ZonMw) from 2001 onward that were finalized before March 1, 2010, were studied. Of the 79 grant proposals, 50 (63%) were intervention studies, 18 (23%) were diagnostic studies, and 6 (8%) were prognostic studies. Subgroups were mentioned in 49 (62%) grant applications and in 53 (67%) publications. In 20 of the 79 projects (25%), the publications were completely in agreement with the grant proposal; that is, subgroups that were prespecified in the grant proposal were reported and no new subgroup analyses were introduced in the publications. Of the 149 prespecified subgroups, 46 (31%) were reported in the final report or scientific publications, and 143 of the 189 (76%) reported subgroups were based on post-hoc findings. For 77% of the subgroup analyses in the publications, there was no mention of whether these were prespecified or post hoc. Justification for subgroup analysis and methods to study subgroups were rarely reported. The authors conclude that there is a large discrepancy between grant applications and final publications regarding subgroup analyses. Both nonreporting prespecified subgroup analyses and reporting post-hoc subgroup analyses are common. More guidance is clearly needed.

Gender differences in research grant applications for pediatric residents.

Recent studies have reported gender differences in research grant applications and funding outcomes for medical school faculty. Our goal was to determine whether similar patterns exist at the resident level and, if so, to explore possible explanations. We conducted a retrospective review of all applications to an internal, mentored research grant fund at a large academic pediatric residency program from 2003 to 2008. We determined whether gender differences existed for application characteristics and outcomes and defined significant predictors of success. During the 5-year period, the fund supported 42 (66%) of 64 applications. Among all applicants, men were more likely than women to hold an advanced research degree. Men requested more money than women and obtained more favorable application scores. Funding success rates were not statistically different between male and female applicants. Among funded applicants, men received higher awards than women, although the percentage of requests funded was the same. In a multiple regression analysis, advanced degree was the significant independent predictor of successful funding outcome. Controlling for advanced degree attenuated the association between gender and timing of application, type of project, dollars requested, and dollars awarded; however, even after controlling for advanced degree, women had inferior grant scores compared with men. Gender differences existed in research grant applications and funding among pediatric residents that mirrored faculty patterns. Among residents, these differences were explained in part by the correlation of male gender with holding an advanced research degree.

Most of the proposed objectives in this grant were achieved during the 3 year duration of the grant and its one year extension. In addition, shortly after initiation of the grant, the discovery of gullies on Mars apparently formed by recent seepage of fluids was announced. Together with partial support from the Astrobiology Institute, I devoted considerable effort during the grant interval into understanding the origin of these gullies because of their astrobiological significance. In addition, longstanding investigations of the environmental conditions of the Early Earth initiated years ago under previous NASA and NSF funding reached fruition and these were presented and published. This report summarizes the significant findings reported during the grant interval. Some of the work initiated during this interval has been completed under the subsequent Exobiology grant and will be reported at the appropriate time.

Under an Army Small Business Innovation Research (SBIR) grant, Symbiotics, Inc. developed a software system that permits users to upgrade products from standalone applications so they can communicate in a distributed computing environment. Under a subsequent NASA SBIR grant, Symbiotics added additional tools to the SOCIAL product to enable NASA to coordinate conventional systems for planning Shuttle launch support operations. Using SOCIAL, data may be shared among applications in a computer network even when the applications are written in different programming languages. The product was introduced to the commercial market in 1993 and is used to monitor and control equipment for operation support and to integrate financial networks. The SBIR program was established to increase small business participation in federal R&D activities and to transfer government research to industry. InQuisiX is a reuse library providing high performance classification, cataloging, searching, browsing, retrieval and synthesis capabilities. These form the foundation for software reuse, producing higher quality software at lower cost and in less time. This paper describes the transformation of the Title V Maternal and Child Health (MCH) Services Block Grant. The Maternal and Child Health Bureau of the Health Resources and Services Administration led a 21-month visioning process to engage input from MCH stakeholders and other national, state and local MCH leaders, families and other partners to improve, innovate, and transform the Title V MCH Services Block Grant. The process has helped inform the development of a new grant guidance for the next 5-year cycle beginning in fiscal year 2016. The triple aims of the transformation are to reduce burden, maintain flexibility, and increase accountability. State reporting burden is reduced by aligning and streamlining the needs assessment, annual report and application, reducing the number of forms States have to fill out, eliminating Health Systems Capacity Indicators, and prepopulating the annual report and application with State data using national data sources. State flexibility is maintained through the needs assessment process whereby State needs and priorities drive the selection of National Performance Measures and State-specific Performance Measures, and the development of State Action Plan and Evidence-based/informed Strategy Measures. Accountability is increased through the new three-tiered performance measurement framework, which will help States tell a more coherent and compelling story about the impact of Title V on the health of the Nation's mothers, children, and families. The ultimate success of the transformation will be measured by how much the transformed Title V program moves the needle in MCH in the States and for the Nation.

The National Institute of Academic Anaesthesia (NIAA) was founded in 2008 to lead a UK strategy for developing academic anaesthesia. We aimed to assess the distribution of applications and quantify the academic returns of NIAA-supported research grants, as this has hitherto not been analysed. We sought data on the baseline characteristics of all grant applicants and recipients. Every grant recipient from 2008 to 2015 was contacted to ascertain the status of their supported research projects. We also examined Google Scholar, Scopus ® database and InCites Journal Citation Reports for citation, author and journal metrics, respectively. In total, 495 research project applications were made, with 150 grants being awarded. Data on 121 out of 150 (80.7%) grant awards, accounting for £3.5 million, were collected, of which 91 completed studies resulted in 140 publications and 2759 citations. The median (IQR [range]) time to first or only publication was 3 (2-4 [0-9]) years. The overall cost per publication was £14,970 (£7457-£24,998 [£2212-£73,755]) and the cost per citation was £1515 (£323-£3785 [£70-£36,182]), with 1 (0-2 [0-8]) publication and 4 (0-25 [0-265]) citations resulting per grant. The impact factor of journals in which publications arose was 4.7 (2.5-6.2 [0-47.8]), with the highest impact arising from clinical and basic science studies, particularly in the fields of pain and peri-operative medicine. Grants were most frequently awarded to clinical and basic science categories of study, but in terms of specialty, critical care medicine and peri-operative medicine received the greatest number of grants. Superficially, there seemed a geographical disparity, with 123 (82%) grants being awarded to researchers in England, London receiving 48 (32%) of these. However, this was in proportion to the number of grant applications received by country or city of application, such that there was no significant difference in overall success rates. There was no significant difference in productivity in terms of publications and citations from grants awarded to each city. The 150 grants were awarded to 107 recipients (identified as the most senior applicant for each grant), 27 of whom received ≥ two grants. Recipients had a median career total of 21 (8-76 [0-254]) publications and 302 (44-1320 [0-8167]) citations, with an h-index of 8 (3-22 [0-54]). We conclude that a key determinant of grant success is simply applying. This is the first study to report the distribution and scholarly output of individual anaesthesia research grants, particularly from a collaborative body such as the NIAA, and can be used as a benchmark to further develop academic anaesthesia in the UK and beyond. © 2018 The Authors. Bias in Research Grant Evaluation Has Dire Consequences for Small Universities.

Federal funding for basic scientific research is the cornerstone of societal progress, economy, health and well-being. There is a direct relationship between financial investment in science and a nation's scientific discoveries, making it a priority for governments to distribute public funding appropriately in support of the best science. However, research grant proposal success rate and funding level can be skewed toward certain groups of applicants, and such skew may be driven by systemic bias arising during grant proposal evaluation and scoring. Policies to best redress this problem are not well established. Here, we show that funding success and grant amounts for applications to Canada's Natural Sciences and Engineering Research Council (NSERC) Discovery Grant program (2011-2014) are consistently lower for applicants from small institutions. This pattern persists across applicant experience levels, is consistent among three criteria used to score grant proposals, and therefore is interpreted as representing systemic bias targeting applicants from small institutions. When current funding success rates are projected forward, forecasts reveal that future science funding at small schools in Canada will decline precipitously in the next decade, if skews are left uncorrected. We show that a recently-adopted pilot program to bolster success by lowering standards for select applicants from small institutions will not erase funding skew, nor will several other post-evaluation corrective measures. Rather, to support objective and robust review of grant applications, it is necessary for research councils to address evaluation skew directly, by adopting procedures such as blind review of research proposals and bibliometric assessment of performance. Such measures will be important in restoring confidence in the objectivity and fairness of science funding decisions. Likewise, small institutions can improve their research success by more strongly supporting productive researchers and developing competitive graduate programming opportunities.

Building the Women's Health Research Workforce: Fostering Interdisciplinary Research Approaches in Women's Health

Background: The Building Interdisciplinary Research Careers in Women's Health (BIRCWH) program is a mentored institutional research career development program developed to support and foster the interdisciplinary research careers of men and women junior faculty in women's health and sex/gender factors. The number of scholars who apply for and receive National Institutes of Health (NIH) research or career development grants is one proximate indicator of whether the BIRCWH program is being successful in achieving its goals. Primary Study Objective: To present descriptive data on one metric of scholar performance—NIH grant application and funding rates. Methods/Design: Grant applications were counted if the start date was 12 months or more after the scholar's BIRCWH start date. Two types of measures were used for the outcome of interest—person-based funding rates and application-based success rates. Main Outcome Measures: Grant application, person funding, and application success rates. Results: Four hundred and ninety-three scholars had participated in BIRCWH as of November 1, 2012. Seventy-nine percent of BIRCWH scholars who completed training had applied for at least one competitive NIH grant, and 64% of those who applied had received at least one grant award. Approximately 68% of completed scholars applied for at least one research grant, and about half of those who applied were successful in obtaining at least one research award. Men and women had similar person funding rates, but women had higher application success rates for RoI grants. Limitations: Data were calculated for all scholars across a series of years; many variables can influence person funding and application success rates beyond the BIRCWH program; and lack of an appropriate comparison group is another substantial limitation to this analysis. Conclusion: Our results suggest that the BIRCWH program has been successful in bridging advanced training with establishing independent research careers for scholars. PMID:24416690

Peer Review of Grant Applications: Criteria Used and Qualitative Study of Reviewer Practices

Background Peer review of grant applications has been criticized as lacking reliability. Studies showing poor agreement among reviewers supported this possibility but usually focused on reviewers’ scores and failed to investigate reasons for disagreement. Here, our goal was to determine how reviewers rate applications, by investigating reviewer practices and grant assessment criteria. Methods and Findings We first collected and analyzed a convenience sample of French and international calls for proposals and assessment guidelines, from which we created an overall typology of assessment criteria comprising nine domains relevance to the call for proposals, usefulness, originality, innovativeness, methodology, feasibility, funding, ethical aspects, and writing of the grant application. We then performed a qualitative study of reviewer practices, particularly regarding the use of assessment criteria, among reviewers of the French Academic Hospital Research Grant Agencies (Programmes Hospitaliers de Recherche Clinique, PHRCs). Semi-structured interviews and observation sessions were conducted. Both the time spent assessing each grant application and the assessment methods varied across reviewers. The assessment criteria recommended by the PHRCs were listed by all reviewers as frequently evaluated and useful. However, use of the PHRC criteria was subjective and varied across reviewers. Some reviewers gave the same weight to each assessment criterion, whereas others considered originality to be the most important criterion (12/34), followed by methodology (10/34) and feasibility (4/34). Conceivably, this variability might adversely affect the reliability of the review process, and studies evaluating this hypothesis would be of interest. Conclusions Variability across reviewers may result in mistrust among grant applicants about the review process. Consequently, ensuring transparency is of the utmost importance. Consistency in the review process could also be improved by providing common definitions for each assessment criterion and uniform requirements for grant application submissions. Further research is needed to assess the feasibility and acceptability of these measures. PMID:23029386
