    Sağlık hakkı temel bir insan hakkıdır. Ancak günümüzde sağlık hakkının hayata geçmesi, sağlık sunumunda gereksinim duyulan kaynakların kısıtlı olması nedeniyle engeller ile karşılaşmaktadır. Ayrıca tıp alanındaki hızlı teknolojik ilerlemeler, tanı ve tedavi imkânlarının her geçen gün gelişmesine yol açmaktadır. Bu durum, sağlık hizmetlerinin sunumunu dağıtıcı adaletin bir süjesi haline getirmektedir. Adalet kavramının tanımlanabilmesi için öncelikle, birey ya da toplulukların bir şeye ilişkin hak talebinde bulunmalarının koşullarını tanımlanmalı daha sonra hak edilenin kim tarafından ve nasıl verileceğinin belirlenmelidir. Etik kuramlar, etik açıdan doğru eyleyebilmek için hangi dayanak noktalarından yola çıkarak hangi değerleri önceleyerek karar vermek gerektiği konusunda kendi paradigmalarını oluşturmuşlardır. Adalet ve sağlık hakkı gibi temel kavramlar, temel etik kuramlar tarafından, o kuramın bağlamı içinde değerlendirilmekte ve anlam kazanmaktadır. Sağlık hakkı bazı etik kuramlar tarafından insan varlığının doğal bir bileşeni olarak tanımlanırken, bazı etik kuramlar tarafından bağlamsal olarak kabul edilmekte bazıları tarafından ise reddedilmekte, adalet kavramı ve bağlantılı olarak adaletin materyal ve formal ilkeleri gibi kavramların içerikleri ve taşıdıkları değerler içinden bakılan etik kuramın paradigmasına bağlı olarak farklıklar içermektedir. Etik kuramların paradigmaları sadece kavramların tanımlanmasında değil, aynı zamanda pratik uygulamalarda da farklı yaklaşımları gerektirmektedir. Bu çalışmada, erdem etiği, faydacı etik kuram, ödev etiği, liberal etik kuram ve kommuniteryan etik kuramın adalet ve sağlık hakkını nasıl kavramsallaştırdıklarını ortaya konmaktır.   2. Monitoring and assessment of radionuclide discharges from Temelín Nuclear Power Plant into the Vltava River (Czech Republic).

    The paper summarizes impacts of the Temelín Nuclear Power Plant (NPP) on the Vltava and Labe River basins. The study is based on the results of long-term monitoring carried out before the plant operation (1989-2000), and subsequently during the plant operation (2001-2005). In the first period, the main objective was to determine background radionuclide levels remaining in the environment after global fallout and due to the Chernobyl accident. A decrease in the concentrations of (90)Sr, (134)Cs and (137)Cs, which was observed before the plant operation, continued also during the subsequent period. Apart from tritium, the results of the observation did not indicate any impacts of the plant on the concentrations of activation and fission products in the hydrosphere. The annual average tritium concentrations in the Vltava River were in agreement with predicted values. The maximum annual average tritium concentration (13.5 Bq L(-1)) was observed in 2004 downstream from the wastewater discharge in the Vltava River at Solenice. Estimated radiation doses for adults due to intakes of river water as drinking water contaminated by tritium are below 0.1 microSv y(-1).

    Developments of radioanalytical methods for determination of radiocarbon in wastewaters from nuclear power plants (NPP) with pressurized light water reactors, which would distinguish between the dissolved organic and inorganic forms have been carried out. After preliminary tests, the method was used to process pilot samples from wastewater outlets from the Temelín and Dukovany NPPs (Czech Republic). The results of analysis of pilot water samples collected in 2015 indicate that the instantaneous 14 C releases into the water streams would be about 7.10 -5 (Temelín) and 4.10 -6 (Dukovany) of the total quantity of the 14 C liberated into the environment.     Dr. Jennifer Temel is an Associate Professor of Medicine at Harvard Medical School and Director of the Cancer Outcomes Research Program at the Massachusetts General Hospital (MGH) Cancer Center.  She is also the Clinical Director of Thoracic Oncology at the MGH Cancer Center and provides oncology care for patients with lung and esophageal cancer. Her research focuses on improving palliative, supportive and end of life care for patients with cancer and their families.  She has received funding from the National Cancer Institute, the National Institute of Nursing Research, the Patient Centered Outcomes Research Institute, and the American Cancer Society to study novel methods of improving the care of cancer patients. One of the main focuses of Dr. Temel’s research involves studying the integration of palliative and oncology care in patients with advanced cancers.  Her work in this area has been published in the New England Journal of Medicine, the Journal of Clinical Oncology, and JAMA.  She was awarded a National Cancer Institute mid-career development award to mentor others in palliative and end of life care in oncology and serves as the co-Principal Investigator on a National Cancer Institute grant to conduct a workshop on methods in supportive oncology research for junior faculty. Dr. Temel recently received the American Academy of Hospice and Palliative Medicine Award for Excellence in Scientific Research, the American Cancer Society Pathfinder Award for her impact on the field of palliative care, and the American Psychosocial Oncology Society Award for Outstanding Education and Training.

    For measuring the fine droplets properties in the wet steam expanding in the steam turbines the light extinction probes are usually used. The paper presents CFD modelling of the extinction probe influence on the wet steam flow field at the measurement position. The aim is to get a basic information about the influence of the flow field deviation on the measured data, in other words, of necessity to correct the measured data. The basic modelling procedure is described, as well as the supposed simplifications and the factor considering the change in the steam density in the measuring slot of the probe. The model is based on the experimental data that were achieved during the developmental measurements in the steam turbine 1090 MW in the power station Temelín. The experimental measurement was done in the cooperation with the Doosan Škoda Power s.r.o.

    SEISMARMARA is a Turkish-French survey carried out in July-October 2001 as a multi-method approach of seismic structure and activity of the Sea of Marmara. This is the segment of the North Anatolian Fault system that continues the one that produced the two destructive earthquakes in 1999 to the East, and is prone to future major earth- quakes as it has experienced in the past. Aims of the programme are to shed light on the regional tectonics and recent evolution at crustal scale, image faults by their structure and seismic activity, and provide a model and reference to improve loca- tion of earthquakes and focal mechanism studies. The programme bases on marine multichannel reflection seismics (MCS), ocean bottom seismometers (OBS) and land stations recording of wide-angle reflection-refraction from the same source, as well as recording of local earthquakes for tomography and stress/strain distribution. The French N/O Le Nadir acquired 4000 km of MCS profiles in the northern Sea of Mar- mara, using a 4.5 km long digital streamer with 360-channels and sources of 8100 cu. in., or 2900 cu. in., provided by a 12-airgun array in single-bubble mode. Navigation safety was provided by a vessel of the Turkish Coast Guards (Sahil Güvenlik), Leg 1 comprises 4 E-W lines and 30 cross-lines in the whole Marmara Trough, leg 2 has 1 been devoted to a very dense grid of lines in the Cinarcik basin and its margins, record- ing over 80 dip-lines at 0.6-0.9 km spacing At sea-bottom 38 OBS, with 3-component sensors and continuous recording over 1 to 2-month in order to also record natural earthquakes were deployed and collected by the Turkish ship MTA Sismik-1. On land the permanent array has been complemented by as many temporary stations, in par- ticular over 30 continuous recording 3-component 2 Hz stations.     Turkey has a short history of forensic medicine compared to the developed countries. Sultan Mahmud II established the first medical school of the Ottoman Empire named as Mekteb-i Tibbiye-i Sahane to provide health services to the army in 1839 [Gok S. Tomorrow, today and yesterday of the forensic medicine. 1st ed. Istanbul: Temel printing office; 1995]. It is also accepted as an important milestone of both medical education and forensic medicine in Turkey [Gok S and Ozen C. History and organization of forensic. 1st ed. Istanbul: Istanbul University Cerrahpasa Medical School Publications; 1982]. The first lecturer of forensic medicine at Mekteb-i Tibbiye-i Sahane was Dr. Charles Ambroise Bernard (C.A.). and he was also the first to perform autopsy in the history of Ottoman Empire [Gok, 1995]. Approximately 40 years after the first forensic medicine lecture in 1879, the Department of Medical Jurisprudence was established as a division of Zabita Tababet-i Adliye (Law Enforcement Office) in Istanbul [Sehsuvaroğlu and Ozen.     During the routine operation, nuclear power plants discharge waste water containing a certain amount of radioactivity, whose main component is the artificial radionuclide tritium. The amounts of tritium released into the environment are kept within the legal requirements, which minimize the noxious effects of radioactivity, but the activity concentration is well measurable in surface water of the recipient. This study compares amount of tritium activity in waste water from nuclear power plants and the tritium activity detected at selected relevant sites of surface water quality monitoring. The situation is assessed in the catchment of the Vltava and Elbe Rivers, affected by the Temelín Nuclear Power Plant as well as in the Jihlava River catchment (the Danube River catchment respectively), where the waste water of the Dukovany Nuclear Power Plant is discharged. The results show a good agreement of the amount of released tritium stated by the power plant operator and the tritium amount detected in the surface water and highlighted the importance of a robust independent monitoring of tritium discharged from a nuclear power plant which could be carried out by water management authorities. The outputs of independent monitoring allow validating the values reported by a polluter and expand opportunities of using tritium as e.g. tracer. Copyright © 2017 Elsevier Ltd. All rights reserved.

  17. Does educating patients about the Early Palliative Care Study increase preferences for outpatient palliative cancer care? Findings from Project EMPOWER.

    Randomized controlled trials, especially the Early Palliative Care Study (Temel et al., 2010), have shown that early outpatient palliative cancer care can improve quality of life for patients with advanced cancer or serious symptoms. However, fear and misconceptions drive avoidance of palliative care. Drawing from an empowerment perspective, we examined whether educating patients about evidence from the Early Palliative Care Study would increase preferences for palliative care. A sample of 598 patients with prostate, breast, lung, colon/rectal, skin, and other cancer diagnoses completed an Internet-mediated experiment using a between-group prepost design. Intervention participants received a summary of the Early Palliative Care Study; controls received no intervention. Participants completed baseline and posttest assessments of preferences of palliative care. Analyses controlled for age, gender, education, cancer type, presence of metastases, time since diagnosis, and baseline preferences. As hypothesized, the intervention had a favorable impact on participants' preferences for outpatient palliative cancer care relative to controls (d = 1.01, p < .001), while controlling for covariates. Intervention participants came to view palliative care as more efficacious (d = 0.79, p < .001) and less scary (d = 0.60, p < .001) and exhibited stronger behavioral intentions to utilize outpatient palliative care if referred (d = 0.60, p < .001). Findings were comparable in patients with metastatic disease, those with less education, and those experiencing financial strain. Educating patients about the Early Palliative Care Study increases preferences for early outpatient palliative care. This research has implications for future studies aimed at improving quality of life in cancer by increasing palliative care utilization.   18. From middle Miocene to late Quaternary spatial and temporal evolution of Cappadocian Volcanism

    Cappadocian Volcanism, Central Turkey was active from Miocene to upper Holocene, originating from varying sources and presents various dynamics. Central Anatolia constitutes a plateau reaching to 1100-1200 meters from the sea level. From Miocene to Quaternary, the volcanism and/or its relationships with local tectonic targeted in numerous works. Those works can be classified as follows: (i) volcanism-tectonic relationship (Pasquare et al, 1988; Toprak and Goncuoglu, 1993; Toprak, 1998, Dhont et al, 1998; Froger et al, 1998), (ii) volcanological, petrological, geochemical works on stratovolcanoes, monogenetic vents, ignimbrites (Batum, 1978; Ercan, 1985; Aydar, 1992; Aydar and Gourgaud, 1993; Aydar et al, 1994; Aydar et al, 1995; Le Pennec et al, 1994; Druitt et al, 1995; Aydar and Gourgaud, 1998; Deniel et al, 1998, Temel, 1998; Kuzucuoglu et al, 1998; Mouralis et al, 2002; Sen et al, 2003) (iii) Geophysical works on the missing calderas (Ongur, 1978; Ekingen, 1982; Froger et al,1998). Cappadocian landscape is made principally of eroded ignimbirites forming fair chimneys. Apart from the ignimbrites, Cappadocia bears several stratovolcanoes (Mt Erciyes, Mt. Hasan) and numerous monogenetic vents (cinder cones, maars, domes) and some andesitic dacitic relicts of lava fields intercalated within the ignimbritic sequence. Although the stratovolcanoes have some historical activities, their initial eruptions occured in Miocene (Kecikalesi stage of Mt Hasan- 13 My), Pliocene (Kocdag stage of Mt Erciyes). The monogenetic vents demonstrate interestingly bi-modal character which is typically found in rifted regions of the world. Origin of this young volcanism is proposed as collision related transitional alkaline-calcalkaline association (Aydar, 1992, Deniel et al, 1998), is also linked to the subduction (Olanca, 1994).     ÖZET Sosyal iletişim ölçeğinin okul öncesi çocuklardaki geçerlik ve güvenirliği Amaç Bu çalışmada Otizm Spektrum Bozukluklarının değerlendirilmesinde kullanılan Sosyal İletişim Ölçeğinin (SİÖ) Türkçe’ye uyarlanarak geçerlik ve güvenirliğinin belirlenmesi amaçlanmıştır. SİÖ, Otizm Tanı Görüşmesi (Autism Diagnostic Interview-ADI) temel alınarak hazırlanmış ve bu görüşmenin kısaltılarak ölçek haline getirilmiş şeklidir. Yöntem 2010–2011 yıllarında polikliniğimize başvuran olgular arasından DSM-IV ölçütlerine göre otizm (n=49) ve Başka Türlü Adlandırılamayan Yaygın Gelişimsel Bozukluk (n=18) tanısı alan 18–60 ay arası çocuklar, Otizm Spektrum Bozukluğu (OSB) grubu olarak çalışmaya alınmıştır. Kontrol grubu olarak aynı yaş grubundaki gelişimsel geriliği olan (25 Mental Retardasyon, 26 konuşma gecikmesi) 51 çocuk ve tipik gelişim gösteren 71 çocuk çalışmaya alınmıştır. Grupların ölçek puanlarının karşılaştırılmasında tek yönlü varyans analizi ve post-hoc Tukey HSD testi kullanılmıştır. Güvenilirlik analizi için iç tutarlılık göstergesi olarak Cronbach alfa değerleri, madde-toplam puan korelasyonları ve test-tekrar test güvenirliği hesaplanmıştır. Faktörlerin belirlenmesi için ana bileşenler yöntemi ve varimax rotasyonu kullanılmıştır. Kesim noktaları, duyarlılık, özgüllük, negatif ve pozitif yordayıcı değerlerin hesaplanması için Receiver Operator Characteristic (ROC) eğrileri kullanılmıştır. Bulgular Ana bileşenler yöntemi ve bunu izleyen varimax rotasyonu sonucunda 3 faktör elde edilmiştir. Bu faktörler sırasıyla İletişim, Karşılıklı Sosyal Etkileşim ve Basmakalıp Davranışlar/Kısıtlı İlgi Alanları olarak adlandırılmıştır.
