Medicinal product evaluation is one of the most important tasks undertaken by government health departments and their regulatory authorities, in every country in the world. The automation and adequate software support are critical tasks that can improve the efficiency and interoperation of regulatory systems across the world. In this paper we propose a software solution that supports the automation of the (i) submission of licensing applications, and (ii) evaluations of submitted licensing applications, according to regulatory authorities' procedures. The novelty of our solution is in allowing licensing applications to be submitted in any country in the world and evaluated according to any evaluation procedure (which can be chosen by either regulatory authorities or pharmaceutical companies). The NCI Genomic Data Commons (GDC) was launched in 2016 and makes available over 4 petabytes (PB) of cancer genomic and associated clinical data to the research community. This dataset continues to grow and currently includes over 14,500 patients. The GDC is an example of a biomedical data commons, which collocates biomedical data with storage and computing infrastructure and commonly used web services, software applications, and tools to create a secure, interoperable, and extensible resource for researchers. The GDC is (i) a data repository for downloading data that have been submitted to it, and also a system that (ii) applies a common set of bioinformatics pipelines to submitted data; (iii) reanalyzes existing data when new pipelines are developed; and (iv) allows users to build their own applications and systems that interoperate with the GDC using the GDC Application Programming Interface (API). The use of substances that inhibit root resorption may be an alternative for cases of unsuccessful reimplants. Hence, the purpose of this study was to test a solution of alendronate, a resorption inhibitor, as an intracanal therapeutic agent for teeth submitted to late reimplantation. Thirty rat maxillary right central incisors were avulsed and kept dry for 30 min. The teeth were instrumented, and the root surfaces treated with 1% hypochlorite solution followed by application of 2% sodium fluoride. Thereafter, the teeth were divided in two groups according to the intracanal dressing: (i) group I, solution of alendronate and (ii) group II, calcium hydroxide paste. Teeth were then reimplanted in their respective sockets. The animals were killed at 15, 30 and 60 days after reimplantation and the samples processed for morphometric and microscopic analysis. The results demonstrated that the solution of alendronate and the calcium hydroxide paste limited the root resorption, yet did not impair its occurrence. It may be concluded that alendronate and calcium hydroxide paste demonstrated similar behavior.

There are many difficulties in undertaking independent clinical research without support from the pharmaceutical industry. In this retrospective observational study, some design characteristics, the clinical trial public register and the publication rate of noncommercial clinical trials were compared to those of commercial clinical trials. A total of 809 applications of drug-evaluation clinical trials were submitted from May 2004 to May 2009 to the research ethics committee of a tertiary hospital, and 16.3% of trials were noncommercial. They were mainly phase IV, multicentre national, and unmasked controlled trials, compared to the commercial trials that were mainly phase II or III, multicentre international, and double-blind masked trials. The commercial trials were registered and published more often than noncommercial trials. More funding for noncommercial research is still needed. The results of the research, commercial or noncommercial, should be disseminated in order not to compromise either its scientific or its social value. © 2018 The British Pharmacological Society.

The aim of this study was to histologically assess the effect of laser therapy (LILT, 660 and 780 nm) on the repair of standardized bone defects on the femur of Wistar albinus rats. The sample was composed of 12 Wistar albinus young adult rats of both genders. Three randomized groups were studied: group I (control, n = 4), group II (LILT, 660 nm, n = 4), and group III (LILT, 780 nm, n = 4). Samples were prepared using a bone defect on the left-side femur surface of the animals, with a total dimension of approximately 3 mm3. Groups II and III were irradiated every 48 h from the second application, where the first dose was given immediately after surgery and the second application came 24 h after surgery. The irradiations were applied transcutaneously at four points around the wound for 14 days. At each point, a dose of 50 J/cm2 (2 J) was given ( s ˜ 0.04 cm2, 40 mW) and the total dose per session was 200 J/cm2 (8 J). The sacrifices were made 15 days after surgery and the specimens were routinely processed to wax, serially cut, stained with an H&E stain, and analyzed under light microscopy. The images were submitted to morphometric analysis using the image segmentation method using the K-means algorithm. The data obtained through the morphometric analysis were submitted to statistical analysis using the Tukey test. The results showed that the group treated with laser therapy in the infrared spectrum resulted in an increase in the repair of bone defects when compared with the group treated with the laser in the red spectrum and control group, which, in turn, had a very similar pattern of repair. A statistical significance ( p < 0.01) was observed when comparing the results of group III and the results of Groups I and II. We concluded that the LILT in the infrared spectrum produced a positive biomodulation effect on the repair of bone defects in the femur of rats.

Effect of two laser photobiomodulation application protocols on the viability of random skin flap in rats

Objective: to identify the best low intensity laser photobiomodulation application site to increase the viability of the cutaneous flap in rats. Methods: 18 male rats (Rattus norvegicus: var. Albinus, Rodentia Mammalia) were randomly distributed into 3 groups (n = 6). Group I (GI) was submitted to simulated laser photobiomodulation, group II (GII) was submitted to the laser photobiomodulation at three points in the flap cranial base, and group III (GIII) was submitted to laser photobiomodulation at twelve points distributed along the flap. All groups were irradiated with an Indium, Galium, Aluminum and Phosphorus diode laser (InGaAlP), 660 nm, with power of 50 mW, total energy of 12 J in continuous emission mode. The treatment started immediately after performing the cranial base random skin flap (dimension of 10X4 cm2 ) and reapplied every 24 hours, with a total of 5 applications. The animals were euthanized after the evaluation of the percentage of necrosis area and the material was collected for histological analysis on the 7th postoperative day. Results: GII animals presented a statistically significant decrease for the necrosis area when compared to the other groups, and a statistically significant increase in the quantification of collagen when compared to the control. We did not observe a statistical difference between the TGFβ and FGF expression in the different groups evaluated. Conclusion: the application of laser photobiomodulation at three points of the flap cranial base was more effective than at twelve points regarding the reduction of necrosis area.

There has been an increase in the number of applications medical students have submitted for the National Residency Matching Program (NRMP). These additional applications are associated with significant costs and may contribute to match inefficiency. We explored if match rates improved in years when an increased number of applications were submitted. We analyzed yearly published data from the NRMP and the Electronic Residency Application Service for 13 specialties. A generalized linear model was used to assess the relationship between the annual match rate and the mean number of applications submitted per applicant, while controlling for the number of positions available and the number of applicants in the given year. Over the last 13 years there has been an increase in the mean number of applications submitted per applicant (P < .001). For the 13 assessed medical specialties, there was no statistically significant relationship between the mean number of applications per applicant per year submitted to the NRMP, and the annual match rate (odds ratios near 1.00 and nonsignificant, P values > .05). There was no improvement in the match rate in years when medical students submitted an increased number of applications. Therefore, it would appear that the applicants do not benefit from the larger number of applications submitted. Further study is required to assess the cost and benefit of these additional applications.

For the 13 assessed medical specialties, there was no statistically significant relationship between the mean number of applications per applicant per year submitted to the NRMP, and the annual match rate (odds ratios near 1.00 and nonsignificant, P values > .05). Conclusions There was no improvement in the match rate in years when medical students submitted an increased number of applications. Therefore, it would appear that the applicants do not benefit from the larger number of applications submitted. Applications for general surgery residency are submitted through the Electronic Residency Application Service (ERAS) beginning in early September. The purpose of this study was to determine whether the date of application submission could be used in the screening of an applicant for general surgery residency. The 2007 ERAS data for an independent program that accepts 2 categorical residents per year was evaluated. International medical graduates were excluded because no international applicants were considered for interviews. Applicants for preliminary positions were also excluded. The remaining graduates from medical schools accredited by the Liaison Committee on Medical Education (LCME) who applied for categorical positions were evaluated based on United States Medical Licensing Examination (USMLE) scores and on medical school performance, as well as on the quality of their personal statements and letters of recommendation. Medical school performance was determined from dean's letters and transcript information, and each applicant was classified as outstanding, average, or poor. The date of application submission was compared with USMLE scores and medical school performance. The lag time to submit an application was also evaluated and compared with whether a student was offered an interview and the assessment of the quality of that interview. Results were evaluated using analysis of variance and the Pearson correlation test to evaluate for significance. A total of 155 applications from LCME-accredited schools for categorical positions were received. The mean lag time to application for students with an outstanding medical school performance was 15.2 +/- 15.5 days compared with 37.4 +/- 26.2 days for poorly performing students (p < 0.01). A negative correlation between USMLE score and the lag time to application was noted (p < 0.01 USMLE I and USMLE II). Applicants offered an interview demonstrated a lag time to submit their application of 19.2 days +/- 21.7 versus 34.0 days +/- 25.8 for applicants not selected to interview (p < 0.01). The results of our study suggest that the date of application submission can provide important screening information about an applicant for general surgery residency. If nearly all high-quality applications are received in September, programs could begin the interview process in early November, which gives students an opportunity to visit more programs and increase their exposure to a broader variety of training options.

Background From 2002 to 2014, the orthopedic surgery residency applicant pool increased by 25% while the number of applications submitted per applicant rose by 69%, resulting in an increase of 109% in the number of applications received per program. Objective This study aimed to identify applicant factors associated with an increased number of applications to orthopedic surgery residency programs. Design An anonymous survey was sent to all applicants applying to the orthopedic surgery residency program at Loyola University. Questions were designed to define the number of applications submitted per respondent as well as the strength of their application. Of 733 surveys sent, 140 (19.1%) responses were received. Setting An academic institution in Maywood, IL. Participants Fourth-year medical students applying to the orthopedic surgery residency program at Loyola University. Results An applicant's perception of how competitive he or she was (applicants who rated themselves as ‘average’ submitted more applications than those who rated themselves as either ‘good’ or ‘outstanding’, p=0.001) and the number of away rotations (those who completed >2 away rotations submitted more applications, p=0.03) were significantly associated with an increased number of applications submitted. No other responses were found to be associated with an increased number of applications submitted. Conclusion Less qualified candidates are not applying to significantly more programs than their more qualified counterparts. The increasing number of applications represents a financial strain on the applicant, given the costs required to apply to more programs, and a time burden on individual programs to screen increasing numbers of applicants. In order to stabilize or reverse this alarming trend, orthopedic surgery residency programs should openly disclose admission criteria to prospective candidates, and medical schools should provide additional guidance for candidates in this process. General Electric Environmental Control Solutions (formerly Alstom Power Environmental Control Systems) set out to complete the Phase 1 award requirements for a Phase II renewal application for their project selected under DOE-FOA-0001190 “Small and Large Scale Pilots for Reducing the Cost of CO 2 Capture and Compression”. The project focus was to implement several improvement concepts utilizing membrane technology at the recipient’s Chilled Ammonia Process (CAP) CO 2 capture large-scale pilot plant. According to the plan presented in the original proposal we have now completed most of the atomic calculations involving collision strengths and rate coefficients for electron impact excitation of C II, N III, and O IV ions. These have been reported in the first two publications appended with this report. We have now moved into the applications phase of the project with the new data being used to analyze the International Ultraviolet Explorer (IUE) observations of a variety of objects, as described in the third publication recently submitted (also appended). The analysis and interpretation of archival data will continue well into the next year with several collaborators that the PI and Co-PI are involved with. In addition, the atomic calculations on Si II have been started.

A cohort of phase I and phase II summary statements for the SBIR grant applications was evaluated to determine the strengths and weaknesses in approved and disapproved applications. An analysis of outcome variables (disapproval or unfunded status) was examined with respect to exposure variables (strengths or shortcomings). Logistic regression models were developed for comparisons to measure the predictive value of shortcomings and strengths to the outcomes. Disapproved phase I results were compared with an earlier 1985 study. Although the magnitude of the frequencies of shortcomings was greater in the present study, the relative rankings within shortcoming class were more alike than different. Also, the frequencies of shortcomings were, with one exception, not significantly different in the two studies. Differences in the summary statement review may have accounted for some differences observed between the 1985 data and results of the present study. Comparisons of Approved/Disapproved and Approved-Unfunded/Funded yielded the following observations. For phase I applicants, a lack of a clearly stated, testable hypothesis, a poorly qualified or described investigative team, and inadequate methodological approaches contributed significantly (in that order) to a rating of disapproval. A critical flaw for phase II proposals was failure to accomplish objectives of the phase I study. Methodological issues also dominate the distinctions in both comparison groups. A clear result of the data presented here and that published previously is that SBIR applicants need continuing assistance to improve the chances of their success. These results should serve as a guide to assist NIH staff as they provide information to prospective applicants focusing on key elements of the application. A continuing review of the SBIR program would be helpful to evaluate the quality of the submitted science.

The Mobile Source Emissions Regulatory Compliance Data Inventory data asset contains measured summary compliance information on light-duty, heavy-duty, and non-road engine manufacturers by model, as well as fee payment data required by Title II of the 1990 Amendments to the Clean Air Act, to certify engines for sale in the U.S. and collect compliance certification fees. Data submitted by manufacturers falls into 12 industries: Heavy Duty Compression Ignition, Marine Spark Ignition, Heavy Duty Spark Ignition, Marine Compression Ignition, Snowmobile, Motorcycle & ATV, Non-Road Compression Ignition, Non-Road Small Spark Ignition, Light-Duty, Evaporative Components, Non-Road Large Spark Ignition, and Locomotive. Title II also requires the collection of fees from manufacturers submitting for compliance certification. Manufacturers submit data on an annual basis, to document engine model changes for certification. A Robust Open Ascending-price Multi-unit Auction Protocol against False-name Bids

This paper develops a new ascending-price multi-unit auction protocol that has following characteristics: (i) it has an open format, (ii) sincere bidding is an equilibrium strategy even if the marginal utilities of each agent can increase and agents can submit false-name bids. False-name bids are bids submitted under fictitious names such as multiple e-mail addresses, which can be done easily in the Internet. This is the first protocol that has these two characteristics. We show that our new protocol outperforms an existing protocol, which satisfies (ii), with respect to the social surplus and the seller's revenue.

A competence- and achievement-based advancement program developed for pharmacists at The Moses H. Cone Memorial Hospital, a 530-bed community teaching hospital, is described. In 1985 pharmacy management recognized the need to provide an incentive program to reward achievements by staff clinical pharmacists, recognize their increased responsibilities, and increase the department's ability to retain qualified practitioners. Adding another rung to the existing administrative career ladder was not desirable, so a new job category, pharmacist II, was created. Employees are evaluated for promotion to pharmacist II on the basis of tenure, performance, and elective professional and service activities. A point system is used to objectively score achievements; to be promoted, an employee must accumulate 50 points during a two-year period. The number of staff pharmacists who can advance is limited not arbitrarily but rather by the application of challenging criteria. Employees are responsible for submitting documentation of their achievements to a review committee, which meets quarterly to evaluate applications and award points. Of 13 applicants since 1985, 10 have been promoted. Because of problems in evaluating the performance of pharmacist II employees, providing them time to conduct research, and incorporating specialty practice areas into the program, plans are being made to institute a more structured career ladder system with separate managerial, clinical, and educational tracks. A competence- and achievement-based advancement program with a single step was an improvement over the former system but lacked the balance and comprehensiveness offered by more structured career ladder programs.

Space Motion Sickness (SMS) is commonly experienced by astronauts and often requires treatment with medications during the early flight days of a space mission. Bioavailability of oral (PO) SMS medications is often low and highly variable; additionally, physiological changes in a microgravity environment exacerbate variability and decrease bioavailability. These factors prompted NASA to develop an intranasal dosage form of scopolamine (INSCOP) suitable for the treatment of SMS. However, to assure safety and efficacy of treatment in space, NASA physicians prescribe commercially available pharmaceutical products only. Development of a pharmaceutical preparation for clinical use must follow distinct clinical phases of testing, phase I through IV to be exact, before it can be approved by the FDA for approval for clinical use. After a physician sponsored Investigative New Drug (IND) application was approved by the FDA, a phase I clinical trial of INSCOP formulation was completed in normal human subjects and results published. The current project includes three phase II clinical protocols for the assessment of pharmacokinetics and pharmacodynamics (PK/PD), efficacy, and safety of INSCOP. Three clinical protocols that were submitted to FDA to accomplish the project objectives: 1) 002-A, a FDA Phase II dose ranging study with four dose levels between 0.1 and 0.4 mg in 12 subjects to assess PK/PD, 2) 002-B, a phase II clinical efficacy study in eighteen healthy subjects to compare efficacy of 0.2 (low dose) and 0.4 mg (high dose) INSCOP for prophylactic treatment of motion-induces (off-axis vertical rotation) symptoms, and (3) 002-C, a phase II clinical study with twelve subjects to determine bioavailability and pharmacodynamics of two doses (0.2 and 0.4 mg) of INSCOP in simulated microgravity, antiorthostatic bedrest. All regulatory procedures were competed that include certification for Good laboratory Procedures by Theradex , clinical documentation, personnel training, selection of clinical research operations contractor, data capturing and management, and annual reporting of results to FDA were successfully completed. Protocol 002-A was completed and sample and data analysis is currently in progress. Protocol 002-B is currently in progress at Dartmouth Hitchcock Medical Center and Protocol 002-C has been submitted to the FDA and will be implemented at the same contractor site as 002-A. An annual report was filed as required by FDA on the results of Protocol 002-A. Once all the three Phase II protocols are completed, a New Drug Administration application will be filed with FDA for Phase III clinical assessment and approval for marketing of the formulation. A commercial vendor will be identified for this phase. This is critical for making this available for treatment of SMS in astronauts and military personnel on duty. Once approved by FDA, INSCOP can be also used by civilian population for motion sickness associated with recreational travel and other ailments that require treatment with anticholinergic drugs.

To determine whether two applicants who misrepresented their accomplishments in applications for gastroenterology fellowships reflected isolated incidents or whether misrepresentation was more wide-spread. Retrospective review of all 236 applications submitted for fellowship in a recent year for confirmation of research experience and cited publications. 138 applicants (58.5%) reported research experience during residency in a U.S. training program. Research activity could not be confirmed for 47 of 138 applicants (34.1%). Fifty-three applicants (22.4%) reported published articles, and 16 of these applicants (30.2%) misrepresented articles. Misrepresentation included citations of nonexistent articles in actual journals, articles in nonexistent journals, or articles noted as "in press." Misrepresentation on applications for gastroenterology fellowships was common. The following steps are recommended: 1) Fellowship programs should require that copies of all publications and letters of acceptance for manuscripts in press be submitted with fellowship applications; 2) applications should contain a statement to be signed by the applicant that the information provided is accurate; 3) persons writing letters of recommendation should verify the information being submitted by applicants; 4) medical students and residents should be taught that embellishment of curricula vitae constitutes misconduct; and 5) institutions and professional organizations should develop policies to deal with this problem.

The purpose of this study was to evaluate the tensile bond strength (TBS) of Panavia F resin cement (PF) applied on dentine pre-treated with ED Primer (ED) and Clearfil Liner Bond 2V (CLB) coated with a layer of low-viscosity composite Protect Liner F (PLF) at 10 min, 24 h and 12 months after curing. The labial surfaces of 60 bovine lower incisors were ground to obtain a flat dentine surface, allowing a demarcation of a 4.0 mm-diameter area with adhesive tape. The teeth were randomly divided in six groups; ED was applied in groups A I, A II and A III and CLB was applied, followed by PLF, in groups B I, B II and B III. A resin composite rod with a wire loop was luted directly to the prepared surface of each group with PF. The specimens of groups A I and B I were submitted to TBS test after 10 min. Groups A II and B II were submitted to TBS test after 24 h storage and groups A III and B III were submitted to TBS test after 12 months storage. Each specimen was inspected by SEM and classified according to the failure mode. Additionally, two representative specimens of each failure mode were sectioned for a composite/dentine interface SEM evaluation. No significant statistical differences were observed among the groups at 10 min and 24 h. Groups A III and B III presented the lowest TBS values (p<0.05) after 12 months storage. PF on resin-coated dentin (PLF) showed the highest TBS values and was statistically different to PF on dentine for all the groups. The fracture pattern was generally cohesive on the adhesive/hybrid layer for groups A I, A II and A III and cohesive on composite resin for B I, B II and B III. The use of a less hydrophilic self-etching system to pre-treat dentine, coating with a low-viscosity composite layer prior luting with resin cement, may provide a protection of the hybridised complex, allowing a dentine seal during the 12 months storage period.

EPA is also proposing to approve numerous revisions to the Oregon SIP that were submitted to EPA by the State of Oregon on October 8, 2008; October 10, 2008; March 17, 2009; June 23, 2010; December 22, 2010 and May 5, 2011. The revisions include updating Oregon's new source review (NSR) rules to be consistent with current Federal regulations and streamlining Oregon's air quality rules by clarifying requirements, removing duplicative rules, and correcting errors.
