Why the offline are offline: a survey of internet use by educated Pakistani adults.

The study reported here explored the reasons for the relatively limited use of the Internet among educated Pakistani adults. A fairly large sample of Internet nonusers from different cities of the Punjab Province with at least secondary school education participated in a survey. Some graduate students and cyber café owners were also interviewed. The results of this study are reported in the main body of this paper, indicating that a variety of cultural, educational, and economic factors are responsible for this state of affairs.

Measuring the performance of Internet companies using a two-stage data envelopment analysis model

In exploring the business operation of Internet companies, few researchers have used data envelopment analysis (DEA) to evaluate their performance. Since the Internet companies have a two-stage production process: marketability and profitability, this study employs a relational two-stage DEA model to assess the efficiency of the 40 dot com firms. The results show that our model performs better in measuring efficiency, and is able to discriminate the causes of inefficiency, thus helping business management to be more effective through providing more guidance to business performance improvement.

vMon-mobile provides wireless connection to the electronic patient record

This work presents the development of a set of tools to help doctors to continuously monitor critical patients. Real-time monitoring signals are displayed via a Web Based Electronic Patient Record (Web-EPR) developed at the Heart Institute. Any computer on the Hospital's Intranet can access the Web-EPR that will open a browser plug-in called vMon. Recently vMon was adapted to wireless mobile devices providing the same real-time visualization of vital signals of its desktop counterpart. The monitoring network communicates with the hospital network through a gateway using HL7 messages and has the ability to export waveforms in real time using the multicast protocol through an API library. A dedicated ActiveX component was built that establishes the streaming of the biomedical signals under monitoring and displays them on an Internet Explorer 5.x browser. The mobile version - called vMon-mobile - will parse the browser window and deliver it to a PDA device connected to a local area network. The result is a virtual monitor presenting real-time data on a mobile device. All parameters and signals acquired from the moment the patient is connected to the monitors are stored for a few days. The most clinically relevant information is added to patient's EPR.

Evolution of Electronic Cigarette Brands From 2013-2014 to 2016-2017: Analysis of Brand Websites

Background The electronic cigarette (e-cigarette) industry has grown in size and organizational complexity in recent years, most notably with the entry of major tobacco companies in 2012 and the proliferation of vape shops. Many brands maintain retail websites that present e-cigarette marketing claims and sell directly to consumers. Understanding of the evolving composition of different types of e-cigarette brand websites is currently underdeveloped. Objective This paper presents how e-cigarette brand websites surveyed in 2013-2014 evolved by 2016-2017, and how the websites run by different types of e-cigarette producers currently differ. Methods In 2016-2017, we revisited 466 e-cigarette brand websites surveyed in 2013-2014, 288 of which were extant, and identified 145 new English-language websites. We compared product designs, marketing claims, and age-based warnings presented by types of e-cigarette producers: major tobacco companies, independent vape shops, and independent internet-only companies. Results Among the 433 websites examined in 2016-2017, 12 were owned by major tobacco companies, 162 operated a physical vape shop, and 259 were internet-only operations. Closed-system product designs were sold by 83% (10/12) of tobacco-owned brands. In comparison, 29.0% (47/162, P<.001) of vape shop and 55.2% (143/259, P=.06) of internet-only brands sold closed-system designs. Compared with vape shop and internet-only brands, tobacco-owned brands offered a smaller set of product models (P values <.001) and a narrower range of flavors (P values <.01), with greater emphasis on the traditional combustible cigarette flavors of tobacco and menthol (P values <.001). Tobacco-owned brands also offered a narrower range of nicotine options than the vape shops (P=.002) and were less likely to offer nicotine-free e-liquid compared with internet-only and vape shop brands (P values <.001). Finally, 83% (10/12) of tobacco-owned brand websites featured age verification pop-up windows. In comparison, only 50.2% (130/259) of internet-only brands (P=.01) and 60.5% (98/162) of vape shop brands (P=.06) featured age verification windows. Websites surveyed in both 2013-2014 and 2016-2017 became more likely to sell open-system mods (P<.001) and sold an increased number of product models (P<.001), flavors (P<.001), and nicotine options (P<.001). Prevalence of several types of claims decreased significantly, including indirect claims regarding smoking cessation (P<.001), claims regarding e-cigarettes as healthier (P<.001), less expensive (P<.001), and usable in more places (P<.001) compared with combustible cigarettes. Conclusions The number of e-cigarette brands has not appeared to increase since 2014, even as website messaging evolved, with brands owned by tobacco companies and vape shops pulling in opposite directions. Brands owned by tobacco companies offered a limited range of e-cigarette products, whereas brands owned by vape shops emphasized a panoply of flavor and nicotine options. Furthermore, the Food and Drug Administration’s regulatory action may influence the types of e-cigarette products offered and the market shares of various companies. PMID:29530840

CIS3/398: Implementation of a Web-Based Electronic Patient Record for Transplant Recipients

Introduction While the "Electronic patient record" (EPR) is a frequently quoted term in many areas of healthcare, only few working EPR-systems are available so far. To justify their use, EPRs must be able to store and display all kinds of medical information in a reliable, secure, time-saving, user-friendly way at an affordable price. Fields with patients who are attended to by a large number of medical specialists over a prolonged period of time are best suited to demonstrate the potential benefits of an EPR. The aim of our project was to investigate the feasibility of an EPR based solely on "of-the-shelf"-software and Internet-technology in the field of organ transplantation. Methods The EPR-system consists of three main elements: Data-storage facilities, a Web-server and a user-interface. Data are stored either in a relational database (Sybase Adaptive 11.5, Sybase Inc., CA) or in case of pictures (JPEG) and files in application formats (e. g. Word-Documents) on a Windows NT 4.0 Server (Microsoft Corp., WA). The entire communication of all data is handled by a Web-server (IIS 4.0, Microsoft) with an Active Server Pages extension. The database is accessed by ActiveX Data Objects via the ODBC-interface. The only software required on the user's computer is the Internet Explorer 4.01 (Microsoft), during the first use of the EPR, the ActiveX HTML Layout Control is automatically added. The user can access the EPR via Local or Wide Area Network or by dial-up connection. If the EPR is accessed from outside the firewall, all communication is encrypted (SSL 3.0, Netscape Comm. Corp., CA).The speed of the EPR-system was tested with 50 repeated measurements of the duration of two key-functions: 1) Display of all lab results for a given day and patient and 2) automatic composition of a letter containing diagnoses, medication, notes and lab results. For the test a 233 MHz Pentium II Processor with 10 Mbit/s Ethernet connection (ping-time below 10 ms) over 2 hubs to the server (400 MHz Pentium II, 256 MB RAM) was used. Results So far the EPR-system has been running for eight consecutive months and contains complete records of 673 transplant recipients with an average follow-up of 9.9 (SD :4.9) years and a total of 1.1 million lab values. Instruction to enable new users to perform basic operations took less than two hours in all cases. The average duration of laboratory access was 0.9 (SD:0.5) seconds, the automatic composition of a letter took 6.1 (SD:2.4) seconds. Apart from the database and Windows NT, all other components are available for free. The development of the EPR-system required less than two person-years. Conclusion Implementation of an Electronic patient record that meets the requirements of comprehensiveness, reliability, security, speed, user-friendliness and affordability using a combination of "of-the-shelf" software-products can be feasible, if the current state-of-the-art internet technology is applied.

Exploring the role of the internet in juvenile prostitution cases coming to the attention of law enforcement.

This exploratory analysis examines the role of the Internet in juvenile prostitution cases coming to the attention of law enforcement. The National Juvenile Prostitution Study (N-JPS) collected information from a national sample of law enforcement agencies about the characteristics of juvenile prostitution cases. In comparison to non-Internet juvenile prostitution cases, Internet juvenile prostitution cases involved younger juveniles and police were more likely to treat juveniles as victims rather than offenders. In addition, these cases were significantly more likely to involve a family or acquaintance exploiter. This analysis suggests that the role of the Internet may impact legal and social service response to juveniles involved in prostitution. In addition, it highlights the need for interventions that acknowledge the vulnerabilities of youth involved in this type of commercial sexual exploitation.

[Interrelations Between Adolescent Problematic Internet Use and Parental Internet Mediation].

Interrelations Between Adolescent Problematic Internet Use and Parental Internet Mediation Everyday life of adolescents and their parents is increasingly characterized by digital media usage (also referred to as "process of mediatization"). In the current study, associations between problematic Internet use of the adolescents (as a possible consequence of the process of mediatization) and parental media education were explored. For this purpose, throughout Germany 1,095 family dyads (an adolescent and a related parent) were investigated with a standardized questionnaire measuring different aspects of parental media education and adolescent problematic Internet use. We conducted two multiple linear regression analyses (dimensional approach) with adolescent problematic Internet use based on self-ratings (model 1, corrected R 2 = 0.18) and parental assessment (model 2, corrected R 2 = 0.24) as response variables. Consistently for self- and parental ratings, adolescent problematic Internet use was statistically significant related to male gender (of the adolescent), a more frequent inconsistent media education (adolescents' and parents' perspective) and a stronger monitoring (parents' perspective). Additionally, we observed associations between the parental rating of adolescent problematic Internet use and a less frequent active and restrictive Internet Mediation (parents' perspective). The findings of the present study show the importance of parental media education for problematic Internet use in adolescence and especially, the role of inconsistent media education should be investigated again in further studies.

The round window region and contiguous areas: endoscopic anatomy and surgical implications.

The round window region is a critical area of the middle ear; the aim of this paper is to describe its anatomy from an endoscopic perspective, emphasizing some structures, the knowledge of which could have important implications during surgery, as well as to evaluate what involvement cholesteatoma may have with these structures. Retrospective review of video recordings of endoscopic ear surgeries and retrospective database review were conducted in Tertiary university referral center. Videos from endoscopic middle ear procedures carried out between June 2010 and September 2012 and stored in a shared database were reviewed retrospectively. Surgeries in which an endoscopic magnification of the round window region and the inferior retrotympanum area was performed intraoperatively were included in the study. Involvement by cholesteatoma of those regions was also documented based on information obtained from the surgical database. Conformation of the tegmen of the round window niche may influence the surgical view of round window membrane. A structure connecting the round window area to the petrous apex, named the subcochlear canaliculus, is described. Cholesteatoma can invade the round window areas in some patients. Endoscopic approaches can guarantee a very detailed view and allow the exploration of the round window region. Exact anatomical knowledge of this region can have important advantages during surgery, since some pathology can invade inside cavities or tunnels otherwise not seen by instrumentation that produces a straight-line view (e.g. microscope).

The round window region is of critical importance in the anatomy of the middle ear. The aim of this paper is to describe its anatomy from an endoscopic point of view, emphasizing structures that have important surgical implications, in particular the fustis and the subcochlear canaliculus. The fustis, a smooth bony structure that forms the floor of the round window region, is a constant and important structure. It seems to indicate the round window membrane and the correct position of scala tympani. A structure connecting the round window region to the petrous apex, named the subcochlear canaliculus, is also described. A retrospective review of video recordings of endoscopic dissection and surgical procedures, carried out between June 2014 and February 2015, was conducted across two Tertiary university referral centers. A total of 42 dissections were analyzed in the study. We observed the fustis in all the cases and we identify two different anatomical conformations. The subcochlear canaliculus was found in 81.0 %, with a pneumatization direct to the petrous apex in 47.7 %. Conformation and limits of the round window niche may influence the surgical view of the round window membrane. Endoscopic approaches allow a very detailed view, which enables a comprehensive exploration of the round window region. Accurate knowledge of the anatomical relationships of this region has important advantages during middle ear surgery.

The Genetic and Environmental Contributions to Internet Use and Associations With Psychopathology: A Twin Study

Excessive internet use has been linked to psychopathology. Therefore, understanding the genetic and environmental risks underpinning internet use and their relation to psychopathology is important. This study aims to explore the genetic and environmental etiology of internet use measures and their associations with internalizing disorders and substance use disorders. The sample included 2,059 monozygotic (MZ) and dizygotic (DZ) young adult twins from the Brisbane Longitudinal Twin Study (BLTS). Younger participants reported more frequent internet use, while women were more likely to use the internet for interpersonal communication. Familial aggregation in ‘frequency of internet use’ was entirely explained by additive genetic factors accounting for 41% of the variance. Familial aggregation in ‘frequency of use after 11 pm’, ‘using the internet to contact peers’, and ‘using the internet primarily to access social networking sites’ was attributable to varying combinations of additive genetic and shared environmental factors. In terms of psychopathology, there were no significant associations between internet use measures and major depression (MD), but there were positive significant associations between ‘frequency of internet use’ and ‘frequency of use after 11 pm’ with social phobia (SP). ‘Using the internet to contact peers’ was positively associated with alcohol abuse, whereas ‘using the internet to contact peers’ and ‘using the internet primarily to access social networking sites’ were negatively associated with cannabis use disorders and nicotine symptoms. Individual differences in internet use can be attributable to varying degrees of genetic and environmental risks. Despite some significant associations of small effect, variation in internet use appears mostly unrelated to psychopathology. PMID:26693596

Small-window parametric imaging based on information entropy for ultrasound tissue characterization

Constructing ultrasound statistical parametric images by using a sliding window is a widely adopted strategy for characterizing tissues. Deficiency in spatial resolution, the appearance of boundary artifacts, and the prerequisite data distribution limit the practicability of statistical parametric imaging. In this study, small-window entropy parametric imaging was proposed to overcome the above problems. Simulations and measurements of phantoms were executed to acquire backscattered radiofrequency (RF) signals, which were processed to explore the feasibility of small-window entropy imaging in detecting scatterer properties. To validate the ability of entropy imaging in tissue characterization, measurements of benign and malignant breast tumors were conducted (n = 63) to compare performances of conventional statistical parametric (based on Nakagami distribution) and entropy imaging by the receiver operating characteristic (ROC) curve analysis. The simulation and phantom results revealed that entropy images constructed using a small sliding window (side length = 1 pulse length) adequately describe changes in scatterer properties. The area under the ROC for using small-window entropy imaging to classify tumors was 0.89, which was higher than 0.79 obtained using statistical parametric imaging. In particular, boundary artifacts were largely suppressed in the proposed imaging technique. Entropy enables using a small window for implementing ultrasound parametric imaging. PMID:28106118

The Internet and Care Initiation by People Living with HIV

Over the past 20 years, people increasingly use the Internet to obtain information, including information about health. Yet, we lack understanding of how people living with HIV (PLHIV) use the Internet in their care and treatment decisions. Interviews with 23 individuals who initiated HIV care at an urban, Midwestern medical center were analyzed to explore how they used the Internet during the process of initiating HIV care. The time frame of initiation of HIV care ranged from less than one month to a delay of three years post-diagnosis. Qualitative content analysis was conducted and revealed that the participants discussed the Internet as a source of information about their diagnosis that influenced their care and treatment decisions. Five predominate themes emerged: 1) The Internet Alerted me to the Possibility of HIV, 2) The Internet Showed me a Solution is Available, 3) The Internet Influenced my Decisions about Care, 4) The Internet Empowered me to Participate in my Treatment Decisions, and 5) The Internet Gave me Hope for my Future. The results suggest that the Internet has the potential to provide information that can profoundly influence PLHIVs acceptance of care and treatment decisions. Clinicians face a new reality in which patients use Internet resources to obtain information and shape opinions about HIV treatment and care initiation decision-making. Guiding PLHIV in their selection of online resources needs to be adopted as one approach to educating and empowering individuals as they cope with their diagnosis and contemplate decisions regarding HIV care and treatment. Understanding children: a qualitative study on health assets of the Internet in Spain.

This research was designed to explore the opinions held by primary school pupils about the Internet as a source of assets for health and well-being. A qualitative study was carried out based on 8 focus groups comprising 64 pupils from 8 primary schools in Spain. Our findings describe the Internet as a tool for learning, communication, fun and health care. In addition, they reveal how children understand influences on health and well-being in relation to their view of the Internet. The results are discussed in terms of the public-health implications of digital literacy, as well as its connection to well-being, especially in relation to health assets. The Internet is an important resource for children's health and well-being, which, through learning, communication, fun and health care, encourages them to make use of it. Digital and health literacy constitutes the foundation required for browsing the Internet in a positive way, as identified by the children interviewed in this study, and especially in relation to the health assets that the Internet can contain.

Lead users' ideas on core features to support physical activity in rheumatoid arthritis: a first step in the development of an internet service using participatory design.

Despite the growing evidence of the benefits of physical activity (PA) in individuals with rheumatoid arthritis (RA), the majority is not physically active enough. An innovative strategy is to engage lead users in the development of PA interventions provided over the internet. The aim was to explore lead users' ideas and prioritization of core features in a future internet service targeting adoption and maintenance of healthy PA in people with RA. Six focus group interviews were performed with a purposively selected sample of 26 individuals with RA. Data were analyzed with qualitative content analysis and quantification of participants' prioritization of most important content. Six categories were identified as core features for a future internet service: up-to-date and evidence-based information and instructions, self-regulation tools, social interaction, personalized set-up, attractive design and content, and access to the internet service. The categories represented four themes, or core aspects, important to consider in the design of the future service: (1) content, (2) customized options, (3) user interface and (4) access and implementation. This is, to the best of our knowledge, the first study involving people with RA in the development of an internet service to support the adoption and maintenance of PA.Participants helped identifying core features and aspects important to consider and further explore during the next phase of development. We hypothesize that involvement of lead users will make transfer from theory to service more adequate and user-friendly and therefore will be an effective mean to facilitate PA behavior change.

Interplay of ICP and IXP over the Internet with power-law features

The Internet is the largest artificial network consisting of billions of IP devices, managed by tens of thousands of autonomous systems (ASes). Due to its importance, the Internet has received much attention and its topological features, mainly in AS-level, have been widely explored from the complex network perspective. However, most of the previous studies assume a homogeneous model in which nodes are indistinguishable in nature. It may be good for a general study of topological structure, but unfortunately it fails to reflect the functionality. The Internet ecology is in fact heterogeneous and highly complex. It consists of various elements such as Internet Exchange Points (IXPs), Internet Content Providers (ICPs), and normal Autonomous System (ASes), realizing different roles in the Internet. In this paper, we propose level-structured network models for investigating how ICP performs under the AS-topology with power-law features and how IXP enhances its performance from a complex network perspective. Based on real data, our results reveal that the power-law nature of the Internet facilitates content delivery not only in efficiency but also in path redundancy. Moreover, the proposed multi-level framework is able to clearly illustrate the significant benefits gained by ICP from IXP peerings.

The Role of Body Image in the Relationship Between Internet Use and Bulimic Symptoms: Three Theoretical Frameworks.

Exposure to traditional media has been associated with bulimic symptoms. However, to date, little is known regarding the effects of Internet exposure. The aim of this study was to explore the relationships between Internet use and bulimic symptoms within the competing frameworks of sociocultural, impression management, and self-objectification theory. A sample of 289 French women aged 18-25 years completed an online questionnaire assessing bulimic symptoms, body dissatisfaction, body image avoidance, self-surveillance, body shame, and weekly Internet use. Bootstrapping analyses revealed that body shame and body image avoidance mediated the effect of weekly Internet use on bulimic symptoms. Furthermore, when entered into a multiple mediation analysis, these two variables provided independent mediation pathways of equal magnitude. The findings support the usefulness of both the self-objectification and impression management frameworks for investigating the relationship between Internet use and bulimic symptoms. Longitudinal research would help to clarify these pathways further.

Exploring the Relationship between Parental Worry about their Children's Health and Usage of an Internet Intervention for Pediatric Encopresis

Objective To investigate whether parental worry about their children's health predicts usage of a pediatric Internet intervention for encopresis. Methods Thirty-nine families with a child diagnosed with encopresis completed a national clinical trial of an Internet-based intervention for encopresis (www.ucanpooptoo.com). Parents rated worry about their children's health, encopresis severity, current parent treatment for depression, and parent comfort with the Internet. Usage indicators were collected while participants utilized the intervention. Results Regression analyses showed that parents who reported higher baseline levels of worry about their children's health showed greater subsequent intervention use (β =.52, p =.002), even after accounting for other plausible predictors. Exploratory analyses indicated that this effect may be stronger for families with younger children. Conclusions Characteristics of individuals using Internet-based treatment programs, such as parental worry about their children's health, can influence intervention usage, and should be considered by developers of Internet interventions. To explore the functional locations of brain regions related to internet addiction (IA)with task-functional magnetic resonance imaging (fMRI). Nineteen college students who had internet game addition and 19 controls accepted the stimuli of videos via computer. The 3.0 Tesla MRI was used to record the Results of echo plannar imaging. The block design method was used. Intragroup and intergroup analysis Results in the 2 groups were obtained. The differences between the 2 groups were analyzed. The internet game videos markedly activated the brain regions of the college students who had or had no internet game addiction. Compared with the control group, the IA group showed increased activation in the right superior parietal lobule, right insular lobe, right precuneus, right cingulated gyrus, and right superior temporal gyrus. Internet game tasks can activate the vision, space, attention and execution center which are composed of temporal occipital gyrus and frontal parietal gyrus. Abnormal brain function and lateral activation of the right brain may exist in IA.

ICT activity in later life: Internet use and leisure activities amongst senior citizens in Finland.

The article examines the relationship between the Internet use and leisure activities amongst Finnish seniors. Traditionally, the young have been the most active users of the Internet and other information and communication technologies (ICTs). In recent years, however, older age groups have increasingly become more interested in ICT, yet a significant proportion of Finnish seniors rarely log on. Using data from a nationally representative survey ( N = 542) conducted in the summer of 2010, we explored the connection between the frequent Internet use and general leisure activity. The basic socio-demographic variables were controlled in the analysis. The findings indicate that the active Internet use in old age has a strong positive correlation with the number of different leisure activities amongst Finnish seniors. As most European societies continue to age, it is clear that future research should focus on the implications and the role the Internet and other new ICTs will play in the everyday life amongst the ageing citizens.
