  1. The Karlsburg Diabetes Management System: translation from research to eHealth application.

    Several telemedicine-based eHealth programs exist, but patient-focused personalized decision support (PDS) is usually lacking. We evaluated the acceptance, efficiency, and cost-effectiveness of telemedicine-assisted PDS in routine outpatient diabetes care. Data are derived from the Diabetiva® program of the German health insurance company BKK TAUNUS. Diabetiva offers telemedicine-based outpatient health care in combination with PDS generated by the Karlsburg Diabetes Management System, KADIS®. This retrospective analysis is based on data from the first year of running KADIS-based PDS in routine diabetes care. Participants were insured persons diagnosed with diabetes and cardiovascular diseases. For final analysis, patients were grouped retrospectively as users or nonusers according to physician acceptance or not (based on questionnaires) of the KADIS-based PDS. A total of 538 patients participated for more than one year in the Diabetiva program. Of these patients, 289 had complete data sets (two continuous glucose monitoring measurements, two or more hemoglobin A1c (HbA1c) values, and a signed questionnaire) and were included in the final data analysis. Of the physicians, 74% accepted KADIS-based PDS, a rate that was clearly related to HbA1c at the beginning of the observation. If KADIS-based PDS was accepted, HbA1c decreased by 0.4% (7.1% to 6.7%). In contrast, rejection of KADIS-based PDS resulted in an HbA1c increase of 0.5% (6.8% to 7.3%). The insurance company revealed an annual cost reduction of about 900 € per participant in the Diabetiva program. KADIS-based PDS in combination with telemedicine has high potential to improve the outcome of routine outpatient diabetes care. © 2010 Diabetes Technology Society.

    Introduction of a health insurance scheme is one of the ways to enhance access to health care services and to protect individuals from catastrophic health expenditures. Little is known on the influence of socio-demographic and social marketing strategies on enrollment and re-enrollment in the Community Health Fund/Tiba Kwa Kadi (CHF/TIKA) in Tanzania. This cross-sectional study employed quantitative methods for data collection between November 2014 and March 2015 in Singida and Shinyanga regions. Relationship between variables was obtained through Chi-square test and multivariate logistic regression. We recruited 496 participants in the study. Majority (92.7%) of participants consented to participate, with 229 (49.8%) and 231 (50.2%) members and non members of CHF/TIKA respectively. Majority (90.9%) were aware of CHF/TIKA. Majority of CHF/TIKA members and non-members (90% and 68.3% respectively) reported health facility-based sensitization as the most common social marketing approach employed to market the CHF/TIKA. The most popular marketing strategies in the country including traditional dances, football games, radio, television, news papers, and mosques/church were reported by few CHF and non CHF members. Multivariate Logistic regression models revealed no significant association between social marketing strategies and enrollment, but only socio-demographics; including marital status (AOR = 2.0, 95% CI 1.1-3.8) and family size (household with ≥ 6 members) (AOR = 1.5, 95% CI 1.0-2.5), were significant factors associated with enrollment/re-enrollment rate. This study indicated that low level of utilization of available social marketing strategies and socio-demographic factors are the barriers for attracting members to join the schemes.   10. Reconnaissance exploration geochemistry in the central Brooks Range, northern Alaska: Implications for exploration of sediment-hosted zinc-lead-silver deposits

    A reconnaissance geochemical survey was conducted in the southern Killik River quadrangle, central Brooks Range, northern Alaska. The Brooks Range lies within the zone of continuous permafrost which may partially inhibit chemical weathering and oxidation. The minus 30-mesh and nonmagnetic heavy-mineral concentrate fractions of sediment samples were chosen as the sample media for the survey so that mechanical rather than chemical dispersion patterns would be enhanced. A total of 263 sites were sampled within the southern half of the Killik River quadrangle at an average sample density of approximately one sample per 12 km2. All samples were submitted for multi-element analyses. In the western and central Brooks Range, several known sediment-hosted Zn-Pb-Ag(-Ba) deposits occur within a belt of Paleozoic rocks of the Endicott Mountains allochthon. Exploration for this type of deposit in the Brook Range is difficult, due to the inherently high background values for Ba, Zn and Pb in shale and the common occurrence of metamorphic quartz-calcite veins, many of which contain traces of sulfide minerals. Stream sediments derived from these sources produce numerous geochemical anomalies which are not necessarily associated with significant mineralization. R-mode factor analysis provides a means of distinguishing between element associations related to lithology and those related to possible mineralization. Factor analysis applied to the multi-element data from the southern Killik River quadrangle resulted in the discovery of two additional Zn-Pb-Ag mineral occurrences of considerable areal extent which are 80-100 km east of any previously known deposit. These have been informally named the Kady and Vidlee. Several lithogeochemical element associations, or factors, and three factors which represent sulfide mineralization were identified: Ag-Pb-Zn (galena and sphalerite) and Fe-Ni-Co-Cu (pyrite ?? chalcopyrite) in the concentrate samples and Cd-Zn-Pb-As-Mn in the sediment

  11. Fluvial dike breaching due to overtopping: how different is it from dam breaching?

When an equilibrium state is reached at the end of the test, the breach centreline orientation is found consistent with the theory of flow over a lateral weir. In the experiments, the boundary condition at the downstream end of the main channel is a lumped representation of river characteristics downstream of the breach section. In real-world conditions, these river characteristics influence the flow partition between the breach and the main channel. Therefore, we tested several downstream boundary conditions (perforated plane, rectilinear weir and sluice gate). For the same inflow discharge and water levels, they lead to significantly different breach geometries. The findings of this research shed light on key mechanisms occurring in fluvial dike breaching, which differ substantially from those in dam breaching. These specific features need to be incorporated in flood risk analyses involving fluvial dike breaching. This research also delivers a unique experimental database of high resolution continuous monitoring of the breach geometry under various flow conditions.   13. Response and resilience of soil microbial communities inhabiting in edible oil stress/contamination from industrial estates.

    Gauging the microbial community structures and functions become imperative to understand the ecological processes. To understand the impact of long-term oil contamination on microbial community structure soil samples were taken from oil fields located in different industrial regions across Kadi, near Ahmedabad, India. Soil collected was hence used for metagenomic DNA extraction to study the capabilities of intrinsic microbial community in tolerating the oil perturbation. Taxonomic profiling was carried out by two different complementary approaches i.e. 16S rDNA and lowest common ancestor. The community profiling revealed the enrichment of phylum "Proteobacteria" and genus "Chromobacterium," respectively for polluted soil sample. Our results indicated that soil microbial diversity (Shannon diversity index) decreased significantly with contamination. Further, assignment of obtained metagenome reads to Clusters of Orthologous Groups (COG) of protein and Kyoto Encyclopedia of Genes and Genomes (KEGG) hits revealed metabolic potential of indigenous microbial community. Enzymes were mapped on fatty acid biosynthesis pathway to elucidate their roles in possible catalytic reactions. To the best of our knowledge this is first study for influence of edible oil on soil microbial communities via shotgun sequencing. The results indicated that long-term oil contamination significantly affects soil microbial community structure by acting as an environmental filter to decrease the regional differences distinguishing soil microbial communities.

    to develop juridical recommendations in order to avoid midwifery medical liability when providing intrapartum care. case law of the past 40 years concerning midwifery medical negligence when assisting labour/delivery in a hospital was analysed. Databases used were Jura and Judit (Belgium), Legifrance, Juricaf and Dalloz (France) and Recht, Rechtspraak (The Netherlands). A minority of cases were retrieved through contacts with insurance companies (only Belgium), lawyers and courts. law suits in Belgium, France and The Netherlands. the 100 analysed cases could be categorised into four types. The judicial assessment was focused on three domains of expertise of the midwife. Most cases involved interpreting fetal monitoring (47%), followed by recognising a specific pathology (32%) and responding to a complication (12%). A fourth type of case concerned exceeding the boundaries of the legal competencies of the midwife (9%). not identifying fetal distress through fetal monitoring was the most common cause of midwifery liability (15/47), followed by not recognising the symptoms of a pathology (10/32), particularly placental abruption and uterine rupture. Also an inaccurate response to complications (3/12) and evidently exceeding the professional competencies involved midwifery liability. achieving cardiotocograph interpretation skills is the first and most important recommendation. In pathologic cases, the midwife should immediately refer to an obstetrician, without any hesitation. The third recommendation is working in a team with the obstetrician. If the midwife has reasonable (evidence-based) doubts about the practice of the obstetrician, she should insist on re-assessment with respect to the boundaries of her competencies. The fourth recommendation concerns practising with knowledge of the client׳s/patient׳s medical record and updating the record with performances and observations.     It has recently been shown within a formal axiomatic framework using a definition of four-momentum based on the Stückelberg-Feynman-Sudarshan-Recami ''switching principle'' that Einstein's relativistic dynamics is logically consistent with the existence of interacting faster-than-light inertial particles. Our results here show, using only basic natural assumptions on dynamics, that this definition is the only possible way to get a consistent theory of such particles moving within the geometry of Minkowskian spacetime. We present a strictly formal proof from a streamlined axiom system that given any slow or fast inertial particle, all inertial observers agree on the value of {m}\\cdot √{|1-v^2|}, where {m} is the particle's relativistic mass and vits speed. This confirms formally the widely held belief that the relativistic mass and momentum of a positive-mass faster-than-light particle must decrease as its speed increases.

    Water pollution and water scarcity are major environmental issues in rural and urban areas. They lead to decline in the quality of water, especially drinking water. Proper qualitative assessment of water is thus necessary to ensure that the water consumed is potable. This study aims to analyze the physicochemical parameters in different sources of water in rural areas and assess the quality of water through a classification system based on BIS and CPCB standards. The classification method has defined water quality in six categories, viz., A, B, C, D, E, and F depending on the levels of physicochemical parameters in the water samples. The proposed classification system was applied to nine villages in Kadi Taluka, Mehsana district of Gujarat. The water samples were collected from borewells, lakes, Narmada Canal, and sewerage systems and were analyzed as per APHA and IS methods. It was observed that most of the physicochemical parameters of Narmada Canal and borewell water fell under class A, thus making them most suitable for drinking. Further, a health camp conducted at Karannagar village, Mehsana revealed no incidents of any waterborne diseases. However, there were certain incidents of kidney stones and joint pain in few villages due to high levels of TDS. Toxic metal analysis in all the water sources revealed low to undetectable concentration of toxic metals such as lead, arsenic, mercury, and cadmium in all the water sources. It is also recommended that the regular treatment of the Narmada Canal water be continued to maintain its excellent quality.

    With this study we investigated the role of nonsteroidal anti-inflammatory drugs (NSAIDs) in human skeletal muscle regeneration. Young men ingested NSAID [1200 mg/d ibuprofen (IBU)] or placebo (PLA) daily for 2 wk before and 4 wk after an electrical stimulation–induced injury to the leg extensor muscles of one leg. Muscle biopsies were collected from the vastus lateralis muscles before and after stimulation (2.5 h and 2, 7, and 30 d) and were assessed for satellite cells and regeneration by immunohistochemistry and real-time RT-PCR, and we also measured telomere length. After injury, and compared with PLA, IBU was found to augment the proportion of ActiveNotch1+ satellite cells at 2 d [IBU, 29 ± 3% vs. PLA, 19 ± 2% (means ± sem)], satellite cell content at 7 d [IBU, 0.16 ± 0.01 vs. PLA, 0.12 ± 0.01 (Pax7+ cells/fiber)], and to expedite muscle repair at 30 d. The PLA group displayed a greater proportion of embryonic myosin+ fibers and a residual ∼2-fold increase in mRNA levels of matrix proteins (all P < 0.05). Endomysial collagen was also elevated with PLA at 30 d. Minimum telomere length shortening was not observed. In conclusion, ingestion of NSAID has a potentiating effect on Notch activation of satellite cells and muscle remodeling during large-scale regeneration of injured human skeletal muscle.—Mackey, A. The DLT-based image processing method uses control points and reference axes, so that no prior knowledge is needed on the position, orientation and intrinsic characteristics of the camera, nor on the laser position. Refraction of the light and laser rays across the water surface needs to be taken into account, because the dike is partially submerged during the experiments. An ad hoc correction is therefore applied using the Snell-Descartes law. For this purpose, planar approximations are used to describe the shape of the water surface. In the presentation, we will discuss the resulting uncertainty and will detail the validation of the developed method based on configurations of known geometry with various complexity. The presented laser profilometry technique allows for a rapid non-intrusive measurement of the dike geometry evolution. It is readily available for laboratory experiments and has proven its performance (Rifai et al. 2017).     Stratiform shale-hosted massive sulfide deposits, sulfidebearing concretions and vein breccias, and barite deposits are widespread in sedimentary rocks of Late Devonian to Permian age in the northern Brooks Range. All of the sulfide-bearing concretions and vein breccias are hosted in mixed continental-marine clastic rocks of the Upper Devonian to Lower Mississippian Endicott Group. The clastic rocks and associated sulfide occurrences underlie chert and shale of Mississippian-Pennsylvanian(?) age that contain large stratiform massive sulfide deposits like that at Red Dog. The relative stratigraphic position of the vein breccias, as well as previously published mineralogical, geochemical, and lead-isotope data, suggest that the vein breccias formed coevally with overlying shale-hosted massive sulfide deposits and that they may represent pathways of oreforming hydrothermal fluids. Barite deposits are hosted either in Mississippian chert and limestone (at essentially the same stratigraphic position as the shale-hosted massive sulfide deposits) or Permian chert and shale. Although most barite deposits have no associated base-metal mineralization, barite occurs with massive sulfide deposits at the Red Dog deposit.Galena and sphalerite from most vein breccias have δ34S values from –7.3 to –0.7‰ (per mil) and –5.1 to 3.6‰, respectively; sphalerite from sulfide-bearing concretions have δ34S values of 0.7 and 4.7‰. This overall range in δ34S values largely overlaps with the range previously determined for galena and sphalerite from shale-hosted massive sulfide deposits at Red Dog and Drenchwater. The Kady vein-breccia occurrence is unusual in having higher δ34S values for sphalerite (12.1 to 12.9‰) and pyrite (11.3‰), consistent with previously published values for the shale-hosted Lik deposit. The correspondence in sulfur isotopic compositions between the stratiform and vein-breccia deposits suggests that they share a common source of reduced sulfur, or

In a recent study, a team of scientists performed complex three-dimensional simulations of coronal jets to answer these questions.Small ExplosionsCoronal jets are relatively small eruptions from the Suns surface, with heights of roughly 100 to 10,000 km, speeds of 10 to 1,000 km/s, and lifetimes of a few minutes to around ten hours. These jets are constantly present theyre emitted even from the quiet Sun, when activity is otherwise low and weve observed them with a fleet of Sun-watching space telescopes spanning the visible, extreme ultraviolet (EUV), and X-ray wavelength bands.A comparison of simulated observations based on the authors model (left panels) to actual EUV and X-ray observations of jets (right panels).   3. 40Ar/39Ar Dating of Zn-Pb-Ag Mineralization in the Northern Brooks Range, Alaska

    The 40Ar/39Ar laser step-heating method potentially can be used to provide absolute ages for a number of formerly undatable, low-temperature ore deposits. This study demonstrates the use of this method by determining absolute ages for Zn-Pb-Ag sediment-hosted massive sulfide deposits and vein-breccia occurrences found throughout a 300-km-long, east-west-trending belt in the northern Brooks Range, Alaska. Massive sulfide deposits are hosted by Mississippian to Pennsylvanian(?) black carbonaceous shale, siliceous mudstone, and lesser chert and carbonate turbidites of the Kuna Formation (e.g., Red Dog, Anarraaq, Lik (Su), and Drenchwater). The vein-breccia occurrences (e.g., Husky, Story Creek, West Kivliktort Mountain, Vidlee, and Kady) are hosted by a deformed but only weakly metamorphosed package of Upper Devonian to Lower Mississippian mixed continental and marine clastic rocks (the Endicott Group) that stratigraphically underlie the Kuna Formation. The vein-breccias are mineralogically similar to, but not spatially associated with, known massive sulfide deposits. The region's largest shale-hosted massive sulfide deposit is Red Dog; it has reserves of 148 Mt grading 16.6 percent zinc, 4.5 percent lead, and 77 g of silver per tonne. Hydrothermally produced white mica in a whole-rock sample from a sulfide-bearing igneous sill within the Red Dog deposit yielded a plateau age of 314.5 Ma. The plateau age of this whole-rock sample records the time at which temperatures cooled below the argon closure temperature of the white mica and is interpreted to represent the minimum age limit for massive sulfide-related hydrothermal activity in the Red Dog deposit. Sulfide-bearing quartz veins at Drenchwater crosscut a hypabyssal intrusion with a maximum biotite age of 337.0 Ma. Despite relatively low sulfide deposition temperatures in the vein-breccia occurrences (162°-251°C), detrital white mica in sandstone immediately adjacent to large vein-breccia zones was partially to

    Giant planets are thought to form in circumstellar disks surrounding young stars, but material may also accrete into a smaller disk around the planet. This circumplanetary disk is important to understand, since it both regulates the late gas accretion and forms the birthplace of future satellites of the planet.Weve yet to detect a circumplanetary disk thus far, because the resolution needed to spot one has been out of reach. Now, however, were entering an era where the disk and its kinematics may be observable with high-powered telescopes (like the Atacama Large Millimeter Array).To prepare for such observations, we need models that predict the basic characteristics of these disks like the mass, temperature, and kinematic properties.
