  1. Development and validation of the knowledge and attitudes regarding antibiotics and resistance (KAAR-11) questionnaire for primary care physicians.

    The aim of this study was to develop a novel, self-administered questionnaire to identify primary-care physicians' knowledge and attitudes regarding antibiotics and resistance (KAAR). The study population comprised primary care physicians. The study was conducted in five phases. Phase I consisted of a systematic review and qualitative focus-group study (n = 33 physicians), in which items were formulated so as to be measured on a continuous, visual analogue scale (VAS); in Phase II, content validation and face validity were evaluated by a panel of experts, which reformulated, added and deleted items; Phase III consisted of a pilot study on a population possessing similar characteristics (n = 15); in Phase IV, we analysed reliability by means of a test-retest study (n = 91) and calculated the intraclass correlation coefficients (ICCs); and in Phase V, we assessed construct validity by applying the known-groups technique, measuring the differences between contrasting groups of physicians formed according to antibiotic prescription quality indicators (group 1, n = 156 versus group 2, n = 191). Following Phases I and II, the questionnaire contained 16 knowledge and attitude items. Participants in the pilot study (Phase III) reported no difficulty. The test-retest study (Phase IV) showed that 11 of the 16 initial knowledge and attitude items yielded an ICC > 0.5, while analysis of known-groups validity (Phase V) showed that 13 of the 16 initial items which assessed knowledge and attitudes discriminated between physicians with good and bad indicators of antibiotics prescription. The final 11 item KAAR questionnaire appears to be valid, reliable and responsive. © The Author 2016.     Muscle biology is important topic in diabetes research. We have reported that a diet with ketogenic amino acids rich replacement (KAAR) ameliorated high-fat diet (HFD)-induced hepatosteatosis via activation of the autophagy system. Here, we found that a KAAR ameliorated the mitochondrial morphological alterations and associated mitochondrial dysfunction induced by an HFD through induction of the AKT/4EBP1 and autophagy signaling pathways in both fast and slow muscles. The mice were fed with a standard HFD (30% fat in food) or an HFD with KAAR (HFD KAAR ). In both the gastrocnemius and the soleus, HFD KAAR ameliorated HFD-impaired mitochondrial morphology and mitochondrial function, characterized by decreased mitofusin 2, optic atrophy 1, peroxisome proliferator-activated receptor (PPAR) γ coactivator-1α and PPARα levels and increased dynamin-related protein 1 levels. The decreased levels of phosphorylated AKT and 4EBP1 in the gastrocnemius and soleus of HFD-fed mice were remediated by HFD KAAR . Furthermore, the HFD KAAR ameliorated the HFD-induced autophagy defects in the gastrocnemius and soleus. These findings suggest that KAAR may be a novel strategy to combat obesity-induced mitochondrial dysfunction, likely through induction of the AKT/4EBP1 and autophagy pathways in skeletal muscle.     The famous 73 ka Toba eruption has produced about 2800 km3 of magma and is considered as one of the largest known eruptions during the Quaternary (Rose and Chesner, 1990). The BAR-9425 piston core collected during the 1994 joint French-Indonesian BARAT Cruise in the north Sumatra Island has recorded the volcano history of Toba from 60 to 100 ka (including the 73 ka Young Toba Tuff (YTT)). Tephra layers within this sediment core have been systematically studied using a combined analysis including stable isotope (δ18O, van der Kaars et al., 2012), high resolution tephrostratigraphic, morphological and a major-trace element studies. Our preliminary results show that not only one major eruption occurred between 60 and 100 ka but probably 11 distinct eruptions occurred. The 11 eruptions display an homogeneous major element composition. The oldest tephra with an estimated age of 101 ka, have a rhyolitic composition and 30% of relative abundance of volcanic glass shards. The other eruptive phases are dated at: 91,5-89,2 ka with a maximum of 16% of volcanic tephra; 85,7-84,8 ka with 64%; 81,8 ka with 22%; 74,4 ka with 43%, 72,3 ka with 89%, 71,4 ka with 92%; 68,9% with 96%; 66,5 ka with 94%; 65,2 ka with 75% and 63,1-60,3 ka with a maximum of 96% of volcanic tephra respectively (ages were calculated with a constant sediment rate of 4,3 cm/ka from data from van der Kaars et al., 2012). Some of these eruptions have had direct effect on regional vegetation as suggested by Van der Kaars et al. (2012) who propose that the gradual expansion of pine cover for the 7000 years following the eruption, is a consequence of the ash deposit from the 89 ka eruption. Our detailed tephrostratigraphy study of Toba eruptions will help to understand the impact on the ecosystems of northern Sumatra and on global climate change. Moreover, we expect to correlate the new tephra layers of Toba volcano to other sites by using AL-ICP-MS traces analyses and to use it as chronological makers.

  8. Quality management and job related factors predicting satisfaction of dental clinic staff in Estonia.

    The aim of the study was to explore the role of managerial style, work environment factors and burnout in determining job satisfaction during the implementation of quality improvement activities in a dental clinic. Quantitative research was carried out using a prestructured anonymous questionnaire to survey 302 respondents in Kaarli Dental Clinic, Estonia. Dental clinic staff assessed job satisfaction, managerial style, work stress and burnout levels through the implementation period of ISO 9000 quality management system in 2003 and annually during 2006-2009. Binary logistic regression was used to explain the impact of satisfaction with management and work organisation, knowledge about managerial activities, work environment and psychosocial stress and burnout on job satisfaction. The response rate limits were between 60% and 89.6%. Job satisfaction increased significantly from 2003 to 2006 and the percentage of very satisfied staff increased from 17 to 38 (p<0.01) over this period. In 2007, the proportion of very satisfied people dropped to 21% before increasing again in 2008-2009 (from 24% to 35%). Binary logistic regression analysis resulted in a model that included five groups of factors: managerial support, information about results achieved and progress to goals, work organisation and working environment, as well as factors related to career, security and planning. The average scores of emotional exhaustion showed significant decrease, correlating negatively with job satisfaction (p<0.05). The implementation of quality improvement activities in the Kaarli Dental Clinic has improved the work environment by decreasing burnout symptoms and increased job satisfaction in staff.

    estimates of primitive upper mantle (PUM). 3 However, even small amounts of Ca-Pv in the crystal assemblage effectively buffer these ratios at values close to the starting composition (e.g. chondritic). Further, based on our new trace element partitioning data, models involving considerable Mg-Pv fractionation generally show poor matches with model PUM. For example, model PUM has sub-chondritic REE/Ti, whereas these ratios increase considerably during Mg-Pv crystallization. Notable exceptions are super-chondritic Zr/Ti, chondritic Sr/Ti, and sub-chondritic Zr/Nb and Sm/Yb ratios, all of which are well matched by considerable Mg-Pv crystallization. Although trace element D's for Ca-Pv are not yet measured quantitatively, the observed affinity of Ca-Pv for REE could conceivably account for the the sub-chondritic REE/Ti ratios in PUM.     The crystal chemistry of a cafarsite sample from the fengitic orthogneisses of the Mt. Leone-Arbola nappe (Lower Penninic), forming the central body of Mount Cervandone and cropping out both in Switzerland and Italy (Alpe Devero area, Verbano-Cusio-Ossola province), was investigated by electron microprobe analysis in wavelength-dispersive mode (EPMA-WDS), single-crystal Raman spectroscopy, and single-crystal X-ray and neutron diffraction at 293 K. The sample of cafarsite of this study was found experimentally to be anhydrous and the chemical formula obtained on the basis of the EPMA-WDS data and structural refinements is the following: Ca1,Ca2 (Ca15.56Na0.44)Σ16 Fe1 (Na0.53Fe2+ 0.17REE0.30)Σ1.00 Mn1,Ti,Fe2 (Ti7.46Fe3+ 4.47Fe2+ 3.20Mn2+ 0.85Al0.11) Σ16.11 As1,As2,As3 (AsO3)28 F F, with the general chemical formula Ca16(Na,Fe2+,REE)(Ti, Fe3+,Fe2+,Mn2+,Al)16(AsO3)28F [or Ca16(Na,Fe2+,REE)(Ti,Fe3+,Al)12(Fe2+,Mn)4(AsO3)28F]. Our experimental findings show that fluorine, which was unconsidered in the previous studies, is a key element. The anhydrous nature of this sample is also confirmed by its Raman spectrum, which does not show any evidence of active bands ascribable to the O-H stretching region. The X-ray and neutron structure refinements provide a structure model that is partially in agreement with the previous experimental findings. The space group (i.e. Pn3) and the unit-cell constant [i.e. 15.9507(4) Å] are conform to the literature data, but the structure of cafarsite, here refined, contains the following building units: three independent AsO3 groups (trigonal pyramids), one CaO6F polyhedron, one CaO8 polyhedron, two independent (Ti,Fe)O6 octahedra, one (Na,Fe,REE)O8 polyhedron, and one (Mn,Fe)O6 octahedron. Connections among polyhedra are mainly due to edge- or vertex-sharing; the AsO3 groups are not connected to each other.

  12. Multidisciplinary studies of the dust storm that affected Sydney in September 2009

    A major dust storm transgressed over southeastern Australia in September 2009 and continued as far as northern Queensland [to the north], New Zealand and New Caledonia [to the east] . We analysed samples of the dust for organic compounds, its microbiological composition, pollen, trace and rare earth elements as well as Sr and Nd isotopes. Grain size analysis was also performed on some of the samples. We also obtained information on the meteorological conditions that led to the large dust plume and its pathway. Our geochemical fingerprinting allowed us to determine the origin of the dust, and this was confirmed by meteorological observations and satellite imagery. As the pathway of the dust plume went over the city of Canberra, located to the southwest of Sydney, we were able to collect samples of dust that fell with rain, and the surprise was that the geochemical composition of the dust varied with time [and dust fall], identifying that as the dust plume transgressed over the landscape, it picked up additional material that was compositionally different from its point of origin. We also compared our data with those obtained from another major dust event that affected Canberra in October 2002, and a number of important differences are noted, particularly with respect of the microbiological composition of the dust, and its chemical composition.     Nuo 2010 m. tarp suaugusiųjų ir paauglių išaugo elektroninių cigarečių vartojimas. Dauguma el. cigarečių vartotojų anksčiau yra rūkę tradicinius tabako gaminius. Didžioji dalis žmonių el. cigaretes suvokia kaip priemonę, padėsiančią mesti rūkyti ar sumažinti surūkomų įprastinių cigarečių kiekį. Elektroninės cigaretės yra prietaisas, susidedantis iš skysčiu užpildytos kasetės, kaitiklio bei baterijos. Nuspaudus mygtuką įjungiamas kaitiklis, kuris šildo skystį, o, šiam tapus garais, jie įkvepiami. Visų rūšių el. skysčių pagrindinės sudedamosios dalys yra glicerolis arba propileno glikolis, papildomai gali būti pridedama nikotino ir / ar aromatų. Skysčiuose taip pat nustatyta įvairių kitų cheminių medžiagų. Ilgalaikio el. cigarečių vartojimo pasekmės sveikatai nėra išaiškintos, manoma, kad jos turėtų būti saugesnės nei rūkyti įprastines cigaretes. Nėra pakankamai ištirtas el. cigarečių skysčiuose esančių medžiagų toksiškumas. Trūksta duomenų apie el. cigarečių, kaip pagalbinės priemonės norint mesti rūkyti, veiksmingumą ir saugumą. Gydytojas norinčiam mesti rūkyti pacientui pirmiausia turėtų pasiūlyti patvirtintas pakaitinės nikotino terapijos ar kitas pagalbines priemones ir užtikrinti, kad pacientas gautų informaciją apie naujausius medikamentus, galinčius jam padėti. Jei pacientas atsisako šių priemonių ir renkasi el. cigaretes, gydytojas turėtų informuoti apie abejotiną jų saugumą ir veiksmingumą. Visuomenėje kyla susirūpinimas, kad el. cigarečių rūkymas gali paskatinti priklausomybę nikotinui ir padidinti norą išbandyti įprastines cigaretes. Nerimą kelia ir neapribotas el. cigarečių vartojimas viešosiose vietose. Nėra žinoma, kokią įtaką aplinkinių sveikatai gali turėti pasyvus garinimas. El. cigarečių reglamentavimas visame pasaulyje skiriasi ir laikui bėgant kinta. PMID:28356791

Furthermore, doctors' perceived interpersonal relationship building was positively related to doctors' satisfaction with the consultation (b=.331, P=.003). In this study, the quality of doctor-patient communication, as indicated by information exchange, interpersonal relationship building, and shared decision making, did not differ significantly between Web-based and face-to-face consultations. Doctors and simulated patients were equally satisfied with both types of consultation medium, and no differences were found in the manner in which participants perceived communicative behavior during these consultations.     Tikslas. Palyginti nėščiųjų ir jaunų negimdžiusių moterų kūno ir krūtų dydžio rodiklius. Medžiaga ir metodika. 2008–2009 m. (I tyrimas) ištirti 82 jaunų negimdžiusių devyniolikos metų amžiaus merginų krūtų ir kūno dydžio rodikliai, jie palyginti su 2013–2015 m. (II tyrimas) Vilniuje tirtų nėščiųjų (pirmuoju nėštumo trečdaliu) tais pačiais kūno stambumo ir krūtų parametrais – ūgiu, svoriu, įvairių kūno dalių apimtimis, odos klostėmis, pasyviąja kūno mase ir krūtų tūriu. Kūno dydžio rodikliams palyginti visos tiriamosios buvo skirstomos į grupes pagal krūtų dydį ir kūno stambumą (procentiliniu metodu). Nėščiosios suskirstytos į gimdančias pirmą kartą ir pakartotinai. Abiejuose tyrimuose naudota standartinė antropometrinė metodika. Krūties tūris apskaičiuotas pagal R. Kramerio ir G. Dexlerio (1981) formulę. Pasyviosios kūno masės (riebalinio audinio) kiekis procentais – pagal odos ir poodžio riebalines klostes naudojant W. E. Sirio (1961), J. H. Wilmorės ir A. R. Behnkės (1970) formules. Rezultatai, nustatyti visų parametrų aprašomosios statistikos, įvairovės ir pasiskirstymo rodikliais, apdoroti naudojant SPSS 22.0 programą. Skirtumai tarp tiriamųjų grupių vertinti pagal Stjudento t kriterijų. Pasirinktas patikimumo lygmuo p < 0,05. Rezultatai. Statistiškai reikšmingai didesni nei jaunų negimdžiusių nėščiųjų grupės moterų rodikliai buvo šie: krūtinės, juosmens, klubų apimties, krūties tūrio ir juosmens klubų indeksas. Nėščių moterų riebalinis audinys kaupėsi daugiau viršutinėje kūno dalyje, nors santykinė ir absoliuti pasyvioji kūno masė tarp grupių skyrėsi nereikšmingai. Lyginant pirmą kartą ir pakartotinai gimdančių su jaunų negimdžiusių moterų rodmenimis paaiškėjo, kad pakartotinai gimdančios moterys turėjo didesnį riebalinio audinio kiekį, o pirmąkart ir jaunų negimdžiusių merginų rodikliai reikšmingai nesiskyrė.
