  1. St. Benedict Sees the Light: Asam's Solar Eclipses as Metaphor

    During the Baroque period, artists worked in a style - encouraged by the Roman Catholic Church and the Council of Trent - that revealed the divine in natural forms and made religious experiences more accessible. Cosmas Damian Asam, painter and architect, and his brother Egid (Aegid) Quirin Asam, sculptor and stuccatore, were the principal exponents of eighteenth-century, southern-German religious decoration and architecture in the grand manner, the Gesamtkunstwerk. Cosmas Damian's visionary and ecstatic art utilized light, both physical and illusionistic, together with images of meteorological and astronomical phenomena, such as solar and lunar eclipses. This paper focuses on his representations of eclipses and demonstrates how Asam was galvanized by their visual, as well as metaphorical power and that he studied a number of them. He subsequently applied his observations in a series of paintings for the Benedictine order that become increasingly astronomically accurate and spiritually profound. From the evidence presented, especially in three depictions of St. Benedict's vision, the artist harnessed his observations to visualize the literary description of the miraculous event in the Dialogues of St. Gregory the Great, traditionally a difficult scene to illustrate, even for Albrecht Dürer. Asam painted the trio at Einsiedeln, Switzerland (1724-27); Kladruby, the Czech Republic (1725-27), where he captured the solar corona and the "diamond-ring effect"; and Weltenburg, Germany (1735), where he also depicted the diamond-ring effect at a total solar eclipse. We conclude that his visualizations were informed by his personal observations of the solar eclipses on 12 May 1706, 22 May 1724, and 13 May 1733. Asam may have also known the eclipse maps of Edmond Halley and William Whiston that were issued in advance.     The Adaptive Safety Analysis and Monitoring (ASAM) system is a hybrid model-based software tool for assisting intelligence analysts to identify terrorist threats, to predict possible evolution of the terrorist activities, and to suggest strategies for countering terrorism. The ASAM system provides a distributed processing structure for gathering, sharing, understanding, and using information to assess and predict terrorist network states. In combination with counter-terrorist network models, it can also suggest feasible actions to inhibit potential terrorist threats. In this paper, we will introduce the architecture of the ASAM system, and discuss the hybrid modeling approach embedded in it, viz., Hidden Markov Models (HMMs) to detect and provide soft evidence on the states of terrorist network nodes based on partial and imperfect observations, and Bayesian networks (BNs) to integrate soft evidence from multiple HMMs. The functionality of the ASAM system is illustrated by way of application to the Indian Airlines Hijacking, as modeled from open sources.

  4. Time to rethink the role of the library in educating doctors: driving information literacy in the clinical environment.

    Can information literacy (IL) be embedded into the curriculum and clinical environment to facilitate patient care and lifelong learning? The Australian School of Advanced Medicine (ASAM) provides competence-based programs incorporating patient-centred care and lifelong learning. ASAM librarians use outcomes-based educational theory to embed and assess IL into ASAM's educational and clinical environments. A competence-based IL program was developed where learning outcomes were linked to current patients and assessed with checklists. Weekly case presentations included clinicians' literature search strategies, results, and conclusions. Librarians provided support to clinicians' literature searches and assessed their presentations using a checklist. Outcome data showed clinicians' searching skills improved over time; however, advanced MEDLINE searching remained challenging for some. Recommendations are provided. IL learning that takes place in context using measurable outcomes is more meaningful, is enduring, and likely contributes to patient care. Competence-based assessment drives learning in this environment.

The off-the-shelf PARTISN multigroup discrete ordinates code is employed to solve the equations underlying the 2nd-ASAM. The accuracy of the results produced using PARTISN is verified by using the results of three test configurations: (1) a homogeneousmore » sphere, for which the response is the exactly known total uncollided leakage, (2) a multiregion two-dimensional (r-z) cylinder, and (3) a two-region sphere for which the response is a reaction rate. For the homogeneous sphere, results for the total leakage as well as for the respective first- and second-order sensitivities are in excellent agreement with the exact benchmark values. For the nonanalytic problems, the results obtained by applying the 2nd-ASAM to compute sensitivities are in excellent agreement with central-difference estimates. The efficiency of the 2nd-ASAM is underscored by the fact that, for the cylinder, only 12 adjoint PARTISN computations were required by the 2nd-ASAM to compute all of the benchmark’s 18 first-order sensitivities and 224 second-order sensitivities, in contrast to the 877 PARTISN calculations needed to compute the respective sensitivities using central finite differences, and this number does not include the additional calculations that were required to find appropriate values of the perturbations to use for the central differences.« less

    The second-order adjoint sensitivity analysis methodology (2nd-ASAM), developed by Cacuci, has been applied by Cacuci to derive second derivatives of a response with respect to input parameters for uncollided particles in an inhomogeneous transport problem. In this memo, we present an analytic benchmark for verifying the derivatives of the 2nd-ASAM. The problem is a homogeneous sphere, and the response is the uncollided total leakage. This memo does not repeat the formulas given in Ref. 2. We are preparing a journal article that will include the derivation of Ref. 2 and the benchmark of this memo.

  9. Preference and intake frequency of high sodium foods and dishes and their correlations with anthropometric measurements among Malaysian subjects

    This study investigated the preference and intake frequency of a list of 15 commonly available high sodium Malaysian foods/dishes, discretionary salt use, and their possible association with demographics, blood pressures and anthropometric measurements among 300 Malaysian university students (114 males, 186 females; 259 ethnic Chinese, 41 Indians; 220 lean, 80 overweight). French fries and instant soup noodle were found to be the most preferred and most frequently consumed salty food, respectively, while salted fish was least preferred and least frequently consumed. Males had a significantly higher intake frequency of at least 6 of the salty foods, but the preference of most salty foods was not significantly different between genders. Ethnic Chinese significantly preferred more and took more frequently traditional and conventional Malaysian foods like asam laksa (a Malaysian salty-sour-spicy noodle in fish stock), salted biscuits and salted vegetable, while Indians have more affinity and frequency towards eating salty Western foods. Body Mass Index was significantly negatively correlated with the intake frequency of canned/packet soup and salted fish while waist circumference was significantly positively correlated with the preference of instant noodle. Also, an increased preference of potato chips and intake frequency of salted biscuits seemed to lead to a decreased WHR. Other than these, all the other overweight/obesity indicators did not seem to fully correlate with the salty food preference and intake frequency. Nevertheless, the preference and intake frequency of asam laksa seemed to be significant negative predictors for blood pressures. Finally, increased preference and intake frequency of high sodium shrimp paste (belacan)-based foods like asam laksa and belacan fried rice seemed to discourage discretionary salt use.     The Asian/American (AS/AM) genotype of dengue virus type 2 (DENV-2) has been evolving in the Americas over the last 30 years, leading to several waves of dengue epidemics and to the emergence of different viral lineages in the region. In this study, we investigate the spatiotemporal dissemination pattern of the DENV-2 lineages at a regional level. We applied phylogenetic and phylogeographic analytical methods to a comprehensive data set of 582 DENV-2 E gene sequences of the AS/AM genotype isolated from 29 different American countries over a period of 30 years (1983 to 2012). Our study reveals that genetic diversity of DENV-2 AS/AM genotype circulating in the Americas mainly resulted from one single founder event and can be organized in at least four major lineages (I to IV), which emerged in the Caribbean region at the early 1980s and then spread and die out with different dynamics. Lineages I and II dominate the epidemics in the Caribbean region during the 1980s and early 1990 s, lineage III becomes the prevalent DENV-2 one in the Caribbean and South America during the 1990 s, whereas lineage IV dominates the epidemics in South and Central America during the 2000s. Suriname and Guyana seem to represent important entry points for DENV-2 from the Lesser Antilles to South America, whereas Venezuela, Brazil and Nicaragua were pointed as the main secondary hubs of dissemination to other mainland countries. Our study also indicates that DENV-2 AS/AM genotype was disseminated within South America following two main routes. The first route hits Venezuela and the western side of the Andes, while the second route mainly hits Brazil and the eastern side of the Andes. The phenomenon of DENV-2 lineage replacement across successive epidemic outbreaks was a common characteristic in all American countries, although the timing of lineage replacements greatly vary across locations.

  18. Housing First/HUD-VASH: Importance, Flaws, and Potential for Transformation: Response to Commentary.

    This paper consists of responses to issues raised in the accompanying Commentary. Our response is organized under three topics: (1) reasons regarding the importance of issues, (2) the need for a comprehensive framework in housing placement, and (3) conceptualization and scientific design (with details on contemporary methods for investigating unanticipated consequences when randomization is no longer feasible). Recurrences to substance use disorder in the American Society of Addiction Medicine (ASAM) housing placement condition are noted within hours or days, with rapid implementation of planned contingencies, and avoidance of enabling. Recurrences in the Housing First/HUD-VASH (HF/H-V) condition escape notice for weeks to months, and continued use is accepted as a core principle of the program. HF/H-V contingencies occur later for major disruptions, leading to discharge. For patients recruited from our clinic, HF/H-V performed poorly when compared to a long-accepted standard of care, the ASAM placement criteria.

    Objectives To develop a nurse-led, urologist-supported model of care for men managed by active surveillance or active monitoring (AS/AM) for localised prostate cancer and provide a formative evaluation of its acceptability to patients, clinicians and nurses. Nurse-led care, comprising an explicit nurse-led protocol with support from urologists, was developed as part of the AM arm of the Prostate testing for cancer and Treatment (ProtecT) trial. Design Interviews and questionnaire surveys of clinicians, nurses and patients assessed acceptability. Setting Nurse-led clinics were established in 9 centres in the ProtecT trial and compared with 3 non-ProtecT urology centres elsewhere in UK. Participants Within ProtecT, 22 men receiving AM nurse-led care were interviewed about experiences of care; 11 urologists and 23 research nurses delivering ProtecT trial care completed a questionnaire about its acceptability; 20 men managed in urology clinics elsewhere in the UK were interviewed about models of AS/AM care; 12 urologists and three specialist nurses working in these clinics were also interviewed about management of AS/AM. Results Nurse-led care was commended by ProtecT trial participants, who valued the flexibility, accessibility and continuity of the service and felt confident about the quality of care. ProtecT consultant urologists and nurses also rated it highly, identifying continuity of care and resource savings as key attributes. Clinicians and patients outside the ProtecT trial believed that nurse-led care could relieve pressure on urology clinics without compromising patient care. Conclusions The ProtecT AM nurse-led model of care was acceptable to men with localised prostate cancer and clinical specialists in urology.     The aims of the project were to determine the glycaemic and insulin responses of non-insulin dependent diabetic patients (NIDDM) to 3 traditional Malaysian meals compared to oral glucose, and to determine whether guar gum would affect these responses. Patients with NIDDM were tested with 75 g oral glucose and three common breakfast meals of the three main ethnic groups of Malaysia. When compared with the oral glucose group, significantly by lower blood glucose responses were seen at 90 and 120 minutes post prandial for nasi lemak (p<0.05) and at 60, 75 and 90 minutes for mee sup (p<0.05). No significant difference was seen for roti telur. There was no significant difference in plasma glucose at any time point of the study when the three test meals were compared with each other. Addition of 5g granulated guar gum mixed with water taken prior to the glucose significantly lowered the plasma glucose at 60, 120 and 150 minutes postprandially (p<0.05). Similarly for the test meals, guar gum significantly lowered plasma glucose concentration between 15 and 45 minutes (p<0.03) postprandial for nasi lemak and between 15 and 30 minutes (p<0.03) for mee sup but not with roti telur. With addition of guar gum, there was no significant change of insulin responses with the three meals but a significant increase was seen at 30 minutes (p<0.02) after ingestion of glucose.

    In 2013, the American Society of Addiction Medicine (ASAM) approved its Standards of Care for the Addiction Specialist Physician. Subsequently, an ASAM Performance Measures Panel identified and prioritized the standards to be operationalized into performance measures. The goal of this study is to describe the process of operationalizing 3 of these standards into quality measures, and to present the initial measure specifications and results of pilot testing these measures in a large health care system. By presenting the process rather than just the end results, we hope to shed light on the measure development process to educate, and also to stimulate debate about the decisions that were made. Each measure was decomposed into major concepts. Then each concept was operationalized using commonly available administrative data sources. Alternative specifications examined and sensitivity analyses were conducted to inform decisions that balanced accuracy, clinical nuance, and simplicity. Using data from the US Veterans Health Administration (VHA), overall performance and variation in performance across 119 VHA facilities were calculated. Three measures were operationalized and pilot tested: pharmacotherapy for alcohol use disorder, pharmacotherapy for opioid use disorder, and timely follow-up after medically managed withdrawal (aka detoxification). Each measure was calculable with available data, and showed ample room for improvement (no ceiling effects) and wide facility-level variability. Next steps include conducting feasibility and pilot testing in other health care systems and other contexts such as standalone addiction treatment programs, and also to study the specification and predictive validity of these measures.

    Area-level public health interventions can be difficult to evaluate using natural experiments. We describe the use of propensity score matching (PSM) to select control local authority areas (LAU) to evaluate the public health impact of alcohol policies for (1) prospective evaluation of alcohol policies using area-level data, and (2) a novel two-stage quasi case-control design. Ecological. Alcohol-related indicator data (Local Alcohol Profiles for England, PHE Health Profiles and ONS data) were linked at LAU level. Six LAUs (Blackpool, Bradford, Bristol, Ipswich, Islington, and Newcastle-upon-Tyne) as sample intervention or case areas were matched to two control LAUs each using PSM. For the quasi case-control study a second stage was added aimed at obtaining maximum contrast in outcomes based on propensity scores. Matching was evaluated based on average standardized absolute mean differences (ASAM) and variable-specific P-values after matching. The six LAUs were matched to suitable control areas (with ASAM < 0.20, P-values >0.05 indicating good matching) for a prospective evaluation study that sought areas that were similar at baseline in order to assess whether a change in intervention exposure led to a change in the outcome (alcohol related harm). PSM also generated appropriate matches for a quasi case-control study--whereby the contrast in health outcomes between cases and control areas needed to be optimized in order to assess retrospectively whether differences in intervention exposure were associated with the outcome. The use of PSM for area-level alcohol policy evaluation, but also for other public health interventions, will improve the value of these evaluations by objective and quantitative selection of the most appropriate control areas.     There are some who suggest that alcoholism and drug abuse are not diseases at all and that they are not consequences of a brain disorder as espoused recently by the American Society of Addiction Medicine (ASAM). Some would argue that addicts can quit on their own and moderate their alcohol and drug intake. When they present to a treatment program or enter the 12 Step Program & Fellowship, many addicts finally achieve complete abstinence. However, when controlled drinking fails, there may be successful alternatives that fit particular groups of individuals. In this expert opinion, we attempt to identify personal differences in recovery, by clarifying the molecular neurobiological basis of each step of the 12 Step Program. We explore the impact that the molecular neurobiological basis of the 12 steps can have on Reward Deficiency Syndrome (RDS) despite addiction risk gene polymorphisms. This exploration has already been accomplished in part by Blum and others in a 2013 Springer Neuroscience Brief. The purpose of this expert opinion is to briefly, outline the molecular neurobiological and genetic links, especially as they relate to the role of epigenetic changes that are possible in individuals who regularly attend AA meetings. It begs the question as to whether “12 steps programs and fellowship” does induce neuroplasticity and continued dopamine D2 receptor proliferation despite carrying hypodopaminergic type polymorphisms such as DRD2 A1 allele. “Like-minded” doctors of ASAM are cognizant that patients in treatment without the “psycho-social-spiritual trio,” may not be obtaining the important benefits afforded by adopting 12-step doctrines. Are we better off with coupling medical assisted treatment (MAT) that favors combining dopamine agonist modalities (DAM) as possible histone-deacetylase activators with the 12 steps followed by a program that embraces either one or the other?     The taxonomy of the causal pathogen of basal stem rot of oil palms, Ganoderma is somewhat problematic at present. In order to determine the genetic distance relationship between G. boninense isolates and non-boninense isolates, a random amplified microsatellites DNA (RAMS) technique was carried out. The result was then compared with interfertility data of G. boninense that had been determined in previous mating studies to confirm the species of G. boninense. Dendrogram from cluster analysis based on UPGMA of RAMS data showed that two major clusters, I and II which separated at a genetic distance of 0.7935 were generated. Cluster I consisted of all the biological species G. boninense isolates namely CNLB, GSDK 3, PER 71, WD 814, GBL 3, GBL 6, OC, GH 02, 170 SL and 348781 while all non-boninense isolates namely G. ASAM, WRR, TFRI 129, G. RES, GJ, and CNLM were grouped together in cluster II. Although the RAMS markers showed polymorphisms in all the isolates tested, the results obtained were in agreement with the interfertility data. Therefore, the RAMS data could support the interfertility data for the identification of Ganoderma isolates.

    To elucidate the genes responsible for constitutive human skin color, we measured the extent of skin pigmentation in the buttock, representative of lifelong non-sun-exposed skin, and conducted a gene mapping study on skin color in an isolated Mongolian population composed of 344 individuals from 59 families who lived in Dashbalbar, Mongolia. The heritability of constitutive skin color was 0.82, indicating significant genetic association on this trait. Through the linkage analysis using 1,039 short tandem repeat (STR) microsatellite markers, we identified a novel genomic region regulating constitutive skin color on 11q24.2 with an logarithm of odds (LOD) score of 3.39. In addition, we also found other candidate regions on 17q23.2, 6q25.1, and 13q33.2 (LOD ≥ 2). Family-based association tests on these regions with suggestive linkage peaks revealed ten and two significant single nucleotide polymorphisms (SNPs) on the linkage regions of chromosome 11 and 17, respectively. We were able to discover four possible candidate genes that would be implicated to regulate human skin color: ETS1, UBASH3B, ASAM, and CLTC.

  16. Towards an improved Land Surface Phenology mapping using a new MODIS product: A case study of Bavarian Forest National Park

    Past work in remote sensing of land surface phenology have mapped vegetation cycles at multiple scales. Much has been discussed and debated about the uncertainties associated with the selection of data, data processing and the eventual conclusions drawn. Several studies do however provide evidence of strong links between different land surface phenology (LSP) metrics with specific ground phenology (GP) (Fisher and Mustard, 2007; Misra et al., 2016). Most importantly the use of high temporal and spatial resolution remote sensing data and ground truth information is critical for such studies. In this study, we use a higher temporal resolution 4 day MODIS NDVI product developed by EURAC (Asam et al., in prep) for the Bavarian Forest National Park during 2002-2015 period and extract various phenological metrics covering different phenophases of vegetation (start of season / sos and end of season / eos). We found the LSP-sos to be more strongly linked to the elevation of the area than LSP-eos which has been cited to be harder to detect (Stöckli et al., 2008). The LSP metrics were also correlated to GP information at 4 different stations covering elevations ranging from approx. 500 to 1500 metres. Results show that among the five dominant species in the area i.e. European ash, Norway spruce, European beech, Norway maple and orchard grass, only particular GP observations for some species show stronger correlations with LSP than others. Spatial variations in the LSP-GP correlations were also observed, with certain areas of the National Park showing positive correlations and others negative. An analysis of temporal trends of LSP also indicates the possibility to detect those areas in the National Park that were affected by extreme events. Further investigations are planned to explain the heterogeneity in the derived LSP metrics using high resolution ground truth data and multivariate statistical analyses.     The Weka Dur gold deposit lies in a cluster of other gold deposits in Badakhshan Province (Ragh district), such as the Kadar, Nesheb Dur, and Rishaw gold occurrences. These gold occurrences lie within a zone of late Hercynian folding and are most likely related to fluids that originated from orogenic processes. The Weka Dur deposit is the largest recorded gold occurrence in Afghanistan and is hosted in Proterozoic mica schist and amphibolite that is intruded by diabase dikes and other intrusive rocks. The tabular orebody is 350 meters (m) long and 2 m wide and can be traced downdip for 110 m. Mineralization consists of ochreous, brecciated schists containing high gold concentrations along gently and steeply dipping fissures. The brecciated rocks grade to 46.7 grams per ton (g/t) silver and contain arsenopyrite, galena, chalcopyrite, and scheelite. Trenches and adits were constructed, mapped, and sampled during the 1960s. Calculated resources are 958.3 kilograms of gold, averaging 4.1 g/t gold.

    In accord with the new definition of addiction published by American Society of Addiction Medicine (ASAM) it is well-known that individuals who present to a treatment center involved in chemical dependency or other documented reward dependence behaviors have impaired brain reward circuitry. They have hypodopaminergic function due to genetic and/or environmental negative pressures upon the reward neuro-circuitry. This impairment leads to aberrant craving behavior and other behaviors such as Substance Use Disorder (SUD). Neurogenetic research in both animal and humans revealed that there is a well-defined cascade in the reward site of the brain that leads to normal dopamine release. This cascade has been termed the “Brain Reward Cascade” (BRC). Any impairment due to either genetics or environmental influences on this cascade will result in a reduced amount of dopamine release in the brain reward site. Manipulation of the BRC has been successfully achieved with neuro-nutrient therapy utilizing nutrigenomic principles. After over four decades of development, neuro-nutrient therapy has provided important clinical benefits when appropriately utilized. This is a review, with some illustrative case histories from a number of addiction professionals, of certain molecular neurobiological mechanisms which if ignored may lead to clinical complications.   19. Treatment of Sleep Disturbance in Alcohol Recovery: A National Survey of Addiction Medicine Physicians

    Sleep disturbance is common among patients in recovery from alcoholism and can precipitate relapse. Though sleep complaints are commonly managed with medication, little is known about their management among recovering alcoholic patients. We performed a postal survey of a self-weighted, random systematic sample of 503 members of the American Society of Addiction Medicine (ASAM) to examine addiction medicine physicians’ medical management of sleep disturbance among patients in early recovery from alcoholism. After 3 mailings, 311 (62%) responded. Of responents, 64% have offered pharmacological treatment to an insomniac, alcoholic patient in the first 3 months after detoxification, but only 22% offered medication to more than half of such patients. Trazodone was the preferred therapy, chosen first by 38% of respondents, followed by other sedating antidepressants (12%), and antihistamines (12%). The mean duration of therapy for trazodone and other sedating antidepressants exceeded one month. Experts in addiction medicine appear reluctant to prescribe medication to sleep-disturbed patients in early recovery from alcoholism. When they do prescribe, trazodone, other sedating antidepressants and antihistamines are favored, despite limited evidence for or against this indication. Although the treatment of disordered sleep among alcoholic patients in early recovery may have merit to prevent relapse, controlled studies of these sleep agents are needed. PMID:12703672

  20. Folate content and availability in Malaysian cooked foods.

    Data on folate availability of Malaysian cooked foods would be useful for estimation of dietary folate intake; however such information is scarce. A total of 53 samples of frequently consumed foods in Malaysia were selected from the Nutrient Composition of Malaysian Foods. Folate content was determined using HPLC method hyphenated with a stainless steel C18 column and ultraviolet detector (lambda = 280 nm). The index of folate availability was defined as the proportion of folate identified as monoglutamyl derivatives from the total folate content. Total folate content of different food samples varied from 30-95 microg/100g fresh weight. Among rice-based dishes, the highest and the lowest total folate was in coconut milk rice (nasi lemak) and ghee rice (nasi minyak), respectively. In noodle dishes, fried rice noodle (kuey teow goreng) and curry noodle (mee kari) had the highest folate contents. The highest index of folate availability was in a flat rice noodle dish (kuey teow bandung) (12.13%), while the lowest was in a festival cake (kuih bakul) (0.13%). Folate content was found to be negatively related to its availability. This study determined folate content and folate availability in commonly consumed cooked foods in Malaysia. The uptake of folate from foods with high folate content may not be necessarily high as folate absorption also depends on the capacity of intestinal deconjugation and the presence of high fibre in the foods.

    Garcinia parvifolia belongs to the same family as mangosteen (Garcinia mangostana), which is known locally in Sabah as "asam kandis" or cherry mangosteen. The present study was conducted to determine the phytochemicals content (total phenolic, flavonoid, anthocyanin, and carotenoid content) and antioxidant and acetylcholinesterase inhibition activity of the flesh and peel of G. parvifolia. All samples were freeze-dried and extracted using 80% methanol and distilled water. For the 80% methanol extract, the flesh of G. parvifolia displayed higher phenolic and flavonoid contents than the peel, with values of 7.2 ± 0.3 mg gallic acid equivalent (GAE)/g and 5.9 ± 0.1 mg rutin equivalent (RU)/g, respectively. Anthocyanins were detected in the peel part of G. parvifolia but absent in the flesh. The peel of G. parvifolia displayed higher total carotenoid content as compared to the flesh part with the values of 17.0 ± 0.3 and 3.0 ± 0.0 mg β-carotene equivalents (BC)/100 g, respectively. The free-radical scavenging, ferric reducing, and acetylcholinesterase inhibition effect of the flesh were higher as compared to the peel in both extracts. These findings suggested that the edible part of G. parvifolia fruit has a potential as a natural source of antioxidant and anti-Alzheimer's agents.

    Garcinia parvifolia belongs to the same family as mangosteen (Garcinia mangostana), which is known locally in Sabah as “asam kandis” or cherry mangosteen. The present study was conducted to determine the phytochemicals content (total phenolic, flavonoid, anthocyanin, and carotenoid content) and antioxidant and acetylcholinesterase inhibition activity of the flesh and peel of G. parvifolia. All samples were freeze-dried and extracted using 80% methanol and distilled water. For the 80% methanol extract, the flesh of G. parvifolia displayed higher phenolic and flavonoid contents than the peel, with values of 7.2 ± 0.3 mg gallic acid equivalent (GAE)/g and 5.9 ± 0.1 mg rutin equivalent (RU)/g, respectively. Anthocyanins were detected in the peel part of G. parvifolia but absent in the flesh. The peel of G. parvifolia displayed higher total carotenoid content as compared to the flesh part with the values of 17.0 ± 0.3 and 3.0 ± 0.0 mg β-carotene equivalents (BC)/100 g, respectively. The free-radical scavenging, ferric reducing, and acetylcholinesterase inhibition effect of the flesh were higher as compared to the peel in both extracts. These findings suggested that the edible part of G. parvifolia fruit has a potential as a natural source of antioxidant and anti-Alzheimer's agents. PMID:24288662

  3. Glycaemic index and glycaemic load of selected popular foods consumed in Southeast Asia.

    The objective of the present study was to determine the glycaemic index (GI) and glycaemic load (GL) values of standard portion sizes of Southeast Asian traditional foods. A total of fifteen popular Southeast Asian foods were evaluated. Of these foods, three were soft drinks, while the other twelve were solid foods commonly consumed in this region. In total, forty-seven healthy participants (eighteen males and twenty-nine females) volunteered to consume either glucose at least twice or one of the fifteen test foods after a 10-12 h overnight fast. Blood glucose concentrations were analysed before consumption of the test food, and 15, 30, 45, 60, 90 and 120 min after food consumption, using capillary blood samples. The GI value of each test food was calculated by expressing the incremental area under the blood glucose response curve (IAUC) value of the test food as a percentage of each participant's average IAUC value, with glucose as the reference food. Among the fifteen foods tested, six belonged to low-GI foods (Ice Green Tea, Beehoon, Pandan Waffle, Curry Puff, Youtiao and Kaya Butter Toast), three belonged to medium-GI foods (Barley Drink, Char Siew Pau and Nasi Lemak), and the other six belonged to high-GI foods (Ice Lemon Tea, Chinese Carrot Cake, Chinese Yam Cake, Chee Cheong Fun, Lo Mai Gai and Pink Rice Cake). The GI and GL values of these traditional foods provide valuable information to consumers, researchers and dietitians on the optimal food choice for glycaemic control. Moreover, our dataset provides GI values of fifteen foods that were not previously tested extensively, and it presents values of foods commonly consumed in Southeast Asia.

  4. American Society of Addiction Medicine (ASAM) National Practice Guideline for the Use of Medications in the Treatment of Addiction Involving Opioid Use.

    The Centers for Disease Control have recently described opioid use and resultant deaths as an epidemic. At this point in time, treating this disease well with medication requires skill and time that are not generally available to primary care doctors in most practice models. Suboptimal treatment has likely contributed to expansion of the epidemic and concerns for unethical practices. At the same time, access to competent treatment is profoundly restricted because few physicians are willing and able to provide it. This "Practice Guideline" was developed to assist in the evaluation and treatment of opioid use disorder, and in the hope that, using this tool, more physicians will be able to provide effective treatment. Although there are existing guidelines for the treatment of opioid use disorder, none have included all of the medications used at present for its treatment. Moreover, few of the existing guidelines address the needs of special populations such as pregnant women, individuals with co-occurring psychiatric disorders, individuals with pain, adolescents, or individuals involved in the criminal justice system. This Practice Guideline was developed using the RAND Corporation (RAND)/University of California, Los Angeles (UCLA) Appropriateness Method (RAM) - a process that combines scientific evidence and clinical knowledge to determine the appropriateness of a set of clinical procedures. The RAM is a deliberate approach encompassing review of existing guidelines, literature reviews, appropriateness ratings, necessity reviews, and document development. For this project, American Society of Addiction Medicine selected an independent committee to oversee guideline development and to assist in writing. American Society of Addiction Medicine's Quality Improvement Council oversaw the selection process for the independent development committee. Recommendations included in the guideline encompass a broad range of topics, starting with the initial evaluation of the patient, the selection of medications, the use of all the approved medications for opioid use disorder, combining psychosocial treatment with medications, the treatment of special populations, and the use of naloxone for the treatment of opioid overdose. Topics needing further research were noted.

    Garcinia atroviridis plant is a commodity that has the potential to become an export commodity in Sumatera Utara. Characterization is an activity in germplasm to determine the morphological properties that can be utilized in differentiating between accessions and assessing the magnitude of genetic diversity. The magnitude of genetic diversity based on morphological properties can support breeding programs. The research was conducted in several areas of G. atroviridis plant, i.e. Gebang, Bahorok, Padang Tualang, Pegajahan, Sei Rampah, Pantai Cermin, and Perbaungan sub-districts in September 2016 with direct observation method. The aim of this research is to explore and characterization between G. atroviridis plant in Langkat and Serdang Berdagai. Parameters morphology characters were observed based on IPGRI, and observation data were analyzed by using SPSS version 21 to obtain the dendogram. The dendogram results showed that there are four groups of kinship relationships on the scale of the spacing (euclidean distance scale) 17. The lowest unequal value (closest kinship) in accessions G3(Bahorok) and G7(Sei Rampah) is 16.328 while the highest value of inequality (farthest kinship) in G5( Padang Tualang) and G6 (Pegajahan) is equal to 54.187. It needs the conservation of G. atroviridis although must be done next observation.

  6. Flood Water Level Mapping and Prediction Due to Dam Failures

    Sembrong dam has undergone overflow failure. Flooding has been reported to hit the town, covering an area of up to Parit Raja, located in the district of Batu Pahat. This study aims to identify the areas that will be affected by flood in the event of a dam failure in Sembrong Dam, Kluang, Johor at a maximum level. To grasp the extent, the flood inundation maps have been generated by using the InfoWorks ICM and GIS software. By using these maps, information such as the depth and extent of floods can be identified the main ares flooded. The flood map was created starting with the collection of relevant data such as measuring the depth of the river and a maximum flow rate for Sembrong Dam. The data were obtained from the Drainage and Irrigation Department Malaysia and the Department of Survey and Mapping and HLA Associates Sdn. Bhd. Then, the data were analyzed according to the established Info Works ICM method. The results found that the flooded area were listed at Sri Lalang, Parit Sagil, Parit Sonto, Sri Paya, Parit Raja, Parit Sempadan, Talang Bunut, Asam Bubok, Tanjung Sembrong, Sungai Rambut and Parit Haji Talib. Flood depth obtained for the related area started from 0.5 m up to 1.2 m. As a conclusion, the flood emanating from this study include the area around the town of Ayer Hitam up to Parit Raja approximately of more than 20 km distance. This may give bad implication to residents around these areas. In future studies, other rivers such as Sungai Batu Pahat should be considered for this study to predict and reduce the yearly flood victims for this area.

    The research was an experimental study in Green Skill Patisserie Course using Project-Based Learning model. It aims to complete the project development of pie named guramnis rainbow pie. Several experiments were carried out to produce a pie dough crust mixed with baked-smashed sweet potato and added with vegetables extract as the food coloring. The experiment method in order to make a better appearance or an attractive shape and to have more nutrition. In addition, the pie was filled with a mixture of sweet and sour gurame as Indonesian traditional food. By applying an organoleptic test to 10 respondents, the result shows that pie dough recipe using flour substituted by baked-smashed sweet potato with 2:1 of a ratio. Coloring pie dough adding extract vegetables (carrots, beets and celery) as color. We found that pie dough has more interesting pie color (90%) and the texture of the pie with a quite level of crispness (60%). Moreover, the pie taste is fairly (70%) and tasty (70%). Nutritional analysis results show that per size, serving guramnis rainbow pie contains energy as much as 81.72 calories, carbohydrates 12.5 grams, fat 2.32 grams and 2.77 grams of protein. The main findings are the pie appearance and taste was different compared to the previous pies because of the pie was served with gurame asam manis as the filling and had flour and cilembu sweet potato as the basic ingredients. The color of guramnis rainbow pie was resulted not only from food coloring but also from vegetables extract namely carrot (orange), bit (red), and salary (green). Thus, it had many benefits for health and adds the nutrition. The researchers recommend a further study in order to make pie dough with baked sweet potato and vegetables extract having an optimal level of crispness.

    In Indonesia, herbal medicines are usually produced by small and medium enterprises which are relatively low in quality control. The purpose of this paper is to report that we have developed a taste sensor system with global selectivity, i.e., electronic tongue (e-tongue) for differentiation of Indonesian herbal medicines. The e-tongue was composed of five kinds of ion selective electrodes as working electrodes, data acquisition system, and pattern recognition system. Each ion selective electrode (ISE) was built by attaching lipid/polymer membrane. For this purpose, the five kinds of membranes were built by mixing lipid, plasticizer (nitrophenyl octyl ether/NPOE), polyvinyl chloride (PVC), and tetrahydrofuran (THF). In this study, we employed five kinds of lipid, namely oleic acid (OA), dioctyl phosphate (DOP), decyl alcohol (DA), dodecylamine (DDC), and trioctyl methyl ammonium chloride (TOMA). In this case, the membranes transform information of taste substances into electric signal. The five kinds of Indonesian herbal medicine were purchased from local supermarket in Yogyakarta, i.e., kunyit asam (made from turmeric and tamarind), beras kencur (made from rice and kencur), jahe wangi (made from ginger and fragrance), sirih wangi (made from betel leaf), and temulawak (made from Javanese ginger). Prior to detecting the taste from the Indonesian herbal medicine samples, each ion selective electrode was tested with five basic taste samples, i.e., for saltiness, sweetness, umami, bitterness, and sourness. All ISEs showed global selectivity to all samples. Furthermore, the array of ISEs showed specific response pattern to each Indonesian herbal medicine. For pattern recognition system, we employed principle component analysis (PCA). As a result, the e-tongue was able to differentiate five kinds of Indonesian herbal medicines, proven by the total variance of first and second principle components is about 93%.     Geologically, Ketaling area consists of a local high considered as flexure margin of Tempino-Kenali Asam Deep in west part and graben in east part also known as East Ketaling Deep. Numerous proven plays were established in Ketaling area with reservoir in early Miocene carbonate and middle Miocene sand. This area underwent several major deformations. Faults are developed widely, yet their geometrical features and mechanisms of formation remained so far indistinct, which limited exploration activities. With new three-dimensional seismic data acquired in 2014, this area evidently interpreted as having strike-slip mechanism. The objective of this study is to examine characteristic of strike slip fault and its affect to hydrocarbon trapping in Ketaling Area. Structural pattern and characteristic of strike slip fault deformation was examined with integration of normal seismic with variance seismic attribute analysis and the mapping of Syn-rift to Post-rift horizon. Seismic flattening on 2D seismic cross section with NW-SE direction is done to see the structural pattern related to horst (paleohigh) and graben. Typical flower structure, branching strike-slip fault system and normal fault in synrift sediment clearly showed in section. An echelon pattern identified from map view as the result of strike slip mechanism. Detail structural geology analysis show the normal fault development which has main border fault in the southern of Ketaling area dipping to the Southeast-East with NE-SW lineament. These faults related to rift system in Ketaling area. NW-SE folds with reactive NE-SW fault which act as hydrocarbon trapping in the shallow zone. This polyphase tectonic formed local graben, horst and inverted structure developed a good kitchen area (graben) and traps (horst, inverted structure). Subsequently, hydrocarbon accumulation potentials such as basement fractures, inverted syn-rift deposit and shallow zone are very interesting to explore in this area.

  11. The petrology of some Indians coals

    Samples from coal seams from all of the major Indonesian coalfields, were examined using reflected white light and reflected flourescence mode microscopy techniques. The coals are rich in vitrinite and have variable, commonly high, contents of liptinite. Inertinite is rare to sparse, with the exception of a few (typically Neogene) coals. Overall, no major differences in coal type exist bewteen Paleogene and Neogene coals. Most of the coals are low in rank ( vitrinite reflectanceoverlineRvmax, 0.30% to 0.57% ). The Neogene coals are typically much lower in rank than the Paleogene coals, and this tendency is most clearly seen within the Kalimantan occurences ( PaleogeneoverlineRvmax 0.53% to 0.67%; Neogene 0.30% to 0.57% ). In Sumatera at Bukit Asam, contact alteration from intrusions causes a marked rise in overlineRv max from the range 0.30% to 0.53% overlineRv max to semi-anthracite (2.6%). Near the intrusions, very high lateral and vertical rank gradients are present. At Ombilin in central west Sumatera, regional rank is relatively high ( overlineRvmax 0.55% to 0.77% ), and similar effects from contact alteration ( overlineRvmax up to 4.6% ) can again be detected. The coals are suitable for power generation. Grindability characteristics should be generally favourable, but the rank of the coals is typically sufficiently low for spontaneous combusion to be a significant problem. The rank of the coals is generally too low for use as a single component charge in conventional coke ovens. Significant reverses exist of coals that could be added as a minor component to imported strongly coals to decrease the foreign exchange cost of coke. The rank and type indicate that yield characteristics should be good for most liquefaction and gasification processes. The coals, and to a lesser extent associated dispersed organic matter, form important source rocks for some of the major natural oil accumulations in Indonesians sedimentary basins.

Indonesian Energy Policy, set up in 1976 were shifting the National Energy Mix to encourage the use of other alternative energy for fulfilling the domestic energy demand. Coal, as it was available in enormous reserve become the most suitable alternative fuel. Indonesian coal mining industry was then gaining a big momentum for its resurrection since it was for long had been overlooked. As the result of reconstruction of old mines, expanding the currentmore » mines and the opening of new mines by foreign investor (Contractors) in Kalimantan, since 1986, ten years after the set up of New National Energy Policy or 45 years after peak production level in the past, 2 million tons of coal production was regained. Afterward the coal production of Indonesian coal mine industry are increasing in an exponential rate of growth. With more than 29 million tons of coal produced in 1994, Indonesia will continue to play greater role in the world coal export market in the future. It is projected that by the year of 1998, Indonesia will rank the 3rd as the world coal exporter next to Australia and South African with around 14% of world market share. In this paper, author would only like to report the current status of Indonesian Coal Briquette Industry of which PT Tambang Batubara Bukit Asam (Persero), PTBA, the state owned coal mining company was being appointed to pioneer the establishment of the first coal briquette industry in Indonesia. Process Technology that being compared here in this paper were based on the technical compliance to specification set by government and the techno-economic evaluation.     Even though the definition of the Joint Director of Laboratories (JDL) "fusion levels" were established in 1987, published 1991, revised in 1999 and 2004, the meaning, effects, control and optimization of interactions among the fusion levels have not as yet been fully explored and understood. Specifically, this is apparent from the abstract JDL definitions of "Levels 2/3 Fusion" - situation and threat assessment (SA/TA), which involve deriving relations among entities, e.g., the aggregation of object states (i.e., classification and location) in SA, while TA uses SA products to estimate/predict the impact of actions/interactions effects on situations taken by the participant entities involved. Given all the existing knowledge in the information fusion and human factors literature, (both prior to and after the introduction of "fusion levels" in 1987) there are still open questions remaining in regard to implementation of knowledge representation and reasoning methods under uncertainty to afford SA/TA. Therefore, to promote exchange of ideas and to illuminate the historical, current and future issues associated with Levels 2/3 implementations, leading experts were invited to present their respective views on various facets of this complex problem. This paper is a retrospective annotated view of the invited panel discussion organized by Ivan Kadar (first author), supported by John Salerno, in order to provide both a historical perspective of the evolution of the state-of-the-art (SOA) in higher-level "Levels 2/3" information fusion implementations by looking back over the past ten or more years (before JDL), and based upon the lessons learned to forecast where focus should be placed to further enhance and advance the SOA by addressing key issues and challenges. In order to convey the panel discussion to audiences not present at the panel, annotated position papers summarizing the panel presentation are included.

  15. Recent progress of geological investigations in Indonesia

    Geologically, the Indonesian archipelago was formed as a result of the interaction and collision of the gigantic crustal blocks, i.e. the Eurasian, Indian, Australian and the Pacific plates. This process caused the formation of extensively distributed ultrabasic rocks in Eastern Indonesia, containing rich mineral resources. In Western Indonesia most ore bodies found are associated with the active volcano-plutonic arc or the stable mass of the Sunda Shelf. There are 60 known Tertiary sedimentary basins in Indonesia and only 36 of them have been "failry" explored, of which 14 basins have had hydrocarbon commercial production. Most of the hydrocarbon exploration and production during the last 100 years have been carried out in Western Indonesia. Many of the "unexplored" basins in Indonesia are located in the offshore areas with water depth over 200 m. Coal and geothermal resources are mostly found in Western Indonesia, particularly Sumatra, Java and Kalimantan. Coal production in 1990 has reached 11 million tons. The steady growth of production was primarily due to the establishment of the coal contract agreement with foreign contractors as well as the re-growth of the State coal mines in Bukit Asam and Sawahlunto, Sumatra. Aside from coal, geothermal is one of the alternative energy resources that have been developed in recent years. From some 16,000 MW resources potential estimated, presently only 140 MW geothermal generating power units have been commercially put on production in Kamojang, West Java. The most important minerals mined in Indonesia are tin (Sn), nickel (Ni), copper (Cu) and bauxite. Most of the gold (Au) and silver (Ag) production are mined in association with copper (Cu) such as those in Tembagapura, Irian Jaya, with the exception of a few epithermal gold mines in other areas in the country. Between 1984 and 1990, Indonesia produced around 1.3-1.5 MMBPD crude oil and condensate plus 1.6-2.2 TSCF natural gas. Most of the natural gas production was

    Mineral dust is a key player in the Earth system and shows diverse impacts on the radiation budget, cloud microphysics, marine and terrestrial ecosystems. Eventually, it also affects our modern way of life. Not only dust emissions from barren or unvegetated soil surfaces like deserts or uncultivated croplands are important sources of airborne mineral dust. Also, during fire events dust is entrained into the atmosphere and appears to contribute noteworthy to the atmospheric dust burden. The underlying process, which drives dust entrainment during fires, is the so-called pyro-convection. The high temperatures in the center of a fire result in an upward motion of the heated air. Subsequently, air flows towards the fire replacing the raising air. The resulting accelerated winds are able to mobilize soil and dust particles up to a size of several millimeters, depending of both the size and the strength of the fire. Several measurements have shown that up to 80% of the mass fraction of the emitted particles during natural or prescribed fires is related to soil or dust particles. The particles are then mixed externally with the combustion aerosols into the convective updraft and were finally inject into altitudes above the planetary boundary layer where they can be distributed and transported over long distances by the atmospheric circulation. To investigate the impacts of such fires on the near-surface wind pattern and the potential for dust emissions via exceeding typical threshold velocities, high resolved Large-Eddy Simulations (LES) with the All Scale Atmospheric Model (ASAM) were executed. In the framework of this study, the influences of different fire properties (fire intensity, size, and shape) and different atmospheric conditions on the strength and extent of fire-related winds and finally their relevance for dust emissions were investigated using sensitivity studies.   17. Costs of day hospital and community residential chemical dependency treatment.

    Patient placement criteria developed by the American Society of Addiction Medicine (ASAM) have identified a need for low-intensity residential treatment as an alternative to day hospital for patients with higher levels of severity. A recent clinical trial found similar outcomes at social model residential treatment and clinically-oriented day hospital programs, but did not report on costs. This paper addresses whether the similar outcomes in the recent trial were delivered with comparable costs, overall and within gender and ethnicity stratum. This paper reports on clients not at environmental risk who participated in a randomized trial conducted in three metropolitan areas served by a large pre-paid health plan. Cost data were collected using the Drug Abuse Treatment Cost Analysis Program (DATCAP). Costs per episode were calculated by multiplying DATCAP-derived program-specific costs by each client's length of stay. Differences in length of stay, and in per-episode costs, were compared between residential and day hospital subjects. Lengths of stay at residential treatment were significantly longer than at day hospital, in the sample overall and in disaggregated analyses. This difference was especially marked among non-Whites. The average cost per week was USD 575 per week at day hospital, versus USD 370 per week at the residential programs. However, because of the longer stays in residential, per-episode costs were significantly higher in the sample overall and among non-Whites (and marginally higher for men). These cost results must be considered in light of the null findings comparing outcomes between subjects randomized to residential versus day hospital programs. The longer stays in the sample overall and for non-White clients at residential programs came at higher costs but did not lead to better rates of abstinence. The short stays in day hospital among non-Whites call into question the attractiveness of day hospital for minority clients. Outcomes and costs

    Introduction Indonesia is an archipelago of more than 17,000 islands that stretches astride the equator for about 5,200 km in southeast Asia (figure 1) and includes major Cenozoic volcano-plutonic arcs, active volcanoes, and various related onshore and offshore basins. These magmatic arcs have extensive Cu and Au mineralization that has generated much exploration and mining in the last 50 years. Although Au and Ag have been mined in Indonesia for over 1000 years (van Leeuwen, 1994), it was not until the middle of the nineteenth century that the Dutch explored and developed major Sn and minor Au, Ag, Ni, bauxite, and coal resources. The metallogeny of Indonesia includes Au-rich porphyry Cu, porphyry Mo, skarn Cu-Au, sedimentary-rock hosted Au, epithermal Au, laterite Ni, and diamond deposits. For example, the Grasberg deposit in Papua has the world's largest gold reserves and the third-largest copper reserves (Sillitoe, 1994). Coal mining in Indonesia also has had a long history beginning with the initial production in 1849 in the Mahakam coal field near Pengaron, East Kalimantan; in 1891 in the Ombilin area, Sumatra, (van Leeuwen, 1994); and in South Sumatra in 1919 at the Bukit Asam mine (Soehandojo, 1989). Total production from deposits in Sumatra and Kalimantan, from the 19thth century to World War II, amounted to 40 million metric tons (Mt). After World War II, production declined due to various factors including politics and a boom in the world-wide oil economy. Active exploration and increased mining began again in the 1980's mainly through a change in Indonesian government policy of collaboration with foreign companies and the global oil crises (Prijono, 1989). This recent coal revival (van Leeuwen, 1994) has lead Indonesia to become the largest exporter of thermal (steam) coal and the second largest combined thermal and metallurgical (coking) coal exporter in the world market (Fairhead and others, 2006).     Ancient Gjirokastra City and Dervician stone deposit, there are located next to each other, in south of Albania, at foots of eastern slope of "Wide Mountain". Building stone it is represented by micritic limestone of white, red and blue colour, formed during Palaeocene-Eocene Period. It contains fossils of rounded forms and it is known and exploited since the ancient times. Argjirokastra, Argjiro's City, appeared since the IV-th Century BC (V.Tola, 2011). Stone City has in its centre Majestic Fortress of a big cruiser view, which is seen from long distance, from all sides of Drinos Valley. "This Majestic Monument of Albanian vigour has an astonishing elegance" has written E. Hoxha (1983). Watching Gjirokastra you will remind "Chronic in Stone", the book of Ismail Kadare, great writer, born in Gjirokastra. All buildings here are of stone. The Large Fortress and high houses as castles are built by stone, and covered by stone tiles. The walls and minarets of religious buildings are of stone. The gates of houses and yards are of engraved stone, protected by metallic nets of artistic forms. The house's walls are built by big stone, while the walls of yards are by small stone of white colour, some times intercalated with lines of red, blue stone. The combination of different colour stone is another one artistic beauty of walls. The roads are paved by black cobblestones of flysch sandstone for protection by slips, some times combined with white limestone mosaics. Steps of houses and roads are by white stone, often reworked masterfully.
