The U.S. Geological Survey, in cooperation with the Kentucky Division of Water, has compiled the reported permitted public water-supply-withdrawal data for Kentucky for 2005. Water-withdrawal data for 1995 and 2000 were previously published in Solley and others (1998) and Hutson and others (2004), respectively. This report is a graphical representation of permitted withdrawals for 1995, 2000, and 2005. Public suppliers that are regulated through the Kentucky Division of Water, Water-Withdrawal Permitting Program, withdrew a total of 496, 525, and 558 million gallons per day (Mgal/d) in 1995, 2000, and 2005, respectively. In 2005, 489 Mgal/d (88 percent) came from surface-water sources, and 69 Mgal/d (12 percent) came from ground-water sources. Small increases and decreases in permitted public water-supply withdrawals can be attributed to population changes. Large increases and decreases can be attributed to merging of supply systems, change(s) in source, or purchases from other counties.

To examine associations between parental limits on soft drinks and purchasing soft drinks from school vending machines and consuming soft drinks among middle school students. Secondary analysis of cross-sectional data from the middle school Youth Risk Behavior Survey. Eight public middle schools in central Kentucky. All sixth- through eighth-grade students in 7 schools and all eighth-grade students in 1 school (n = 4,049). Self-reported parental limits on soft drink intake, school vending machine soft drink purchases, soft drink consumption, and control variables. Chi-square and binary logistic regression analyses. Compared to students with no parental limits on soft drink consumption, students with the strictest limits were less likely to purchase soft drinks from school vending machines and consume soft drinks; conversely, students with minor parental limitations were more likely to consume soft drinks. The odds of consuming soft drinks were nearly 4 times greater when students purchased soft drinks from school vending machines than when they did not. Further research may enhance understanding of the influence of varying degrees of parental limitations. Efforts to reduce children's soft drink consumption may be augmented by policies limiting soft drink purchases in schools. Copyright 2010 Society for Nutrition Education. Published by Elsevier Inc. Overweight and poor nutrition of children in the United States are becoming issues of increasing concern for public health. Dietary patterns of U.S. children indicate they are consuming too few fruits and vegetables and too many foods high in fat and sugar. Contributing to this pattern of food consumption is snacking, which is reported to be on the increase among adults and children alike. One place where snacking is under increased scrutiny, and where it is being increasingly criticized, is in U.S. schools, where snack foods are often sold to supplement inadequate budgets. This article takes a biocultural approach to understanding the nutritional status of elementary school children in a rural community in eastern Kentucky. It pays particular attention to the ways in which the school's nutrition environment shapes overweight and nutritional status for many of the children, focusing on the sale of snack foods and the reasons behind the principal's decision to sell snack foods in the school.

The effect of parental involvement on problematic social behaviors among school-age children in Kentucky.

This study examines the associations among parental active involvement and healthy role modeling behavior with social behavior among children in Kentucky and the nation. Data from the 2007 National Survey of Children's Health was used, limited to children 6-17 years old. The dependent variable was a composite measure of problematic social behavior. Independent variables included parental involvement, parental healthy role modeling, and demographic variables. Chi square tests of independence were completed for bivariate analyses and multivariable logistic regression models were developed for Kentucky and the nation. The prevalence of problematic social behaviors in children was 10.4 % in Kentucky and 8.8 % in the nation. The parents of children in Kentucky who often exhibited problematic social behavior reported poor parent-child communication (50.4 %), not coping well with parenthood (56.5 %), parental aggravation (48.3 %), and less emotional help with parenting (9.1 %). The factor with the largest magnitude of association in Kentucky (adjusted odds ratio [AOR] = 6.2; 95 % confidence interval [CI]: 1.6, 24.5) and the nation (AOR = 4.8; 95 % CI: 3.3, 7.0) was observed for whether or not the parent communicated well with the child. Additional factors associated with problematic social behavior among children in Kentucky were living in a single parent, mother-led household, and having a parent with fair or poor mental health. Public health programs that target factors addressing the parent-child dyad, parent-child communication, and model healthy relationships may reduce the occurrence of problematic social behavior in 6-17-year-old children in Kentucky.

Borrowing from business, quality improvement programs, and strategic planning principles, environmental scanning is gaining popularity in public health practice and research and is advocated as an assessment and data collection tool by federal funding agencies and other health-related organizations. Applicable to a range of current and emerging health topics, environmental scans - through various methods - assess multiple facets of an issue by engaging stakeholders who can ask or answer research questions, exploring related policy, critiquing published and gray literature, collecting and analyzing qualitative and quantitative data in both primary and secondary forms, disseminating findings to internal and external stakeholders, and informing subsequent planning and decision making. To illustrate the environmental scanning process in a public health setting and showcase its value to practitioners in the field, we describe a federally funded environmental scan for a human papillomavirus vaccination project in Kentucky.

The U.S. Geological Survey (USGS) is the principal Federal water-resources data collection and investigation agency. Through the Water Resources Division District Office in Kentucky, the USGS investigates the occurrence, distribution, quantity, movement, and chemical and biological quality of surface and ground water in the State. The mission of this program is to collect, interpret, and publish information on water resources. Almost all research and data collection is a cooperative effort in which planning and financial support are shared by State and local agencies and governments. Other activities are funded by other Federal agencies or by direct Congressional appropriation. This report is intended to inform the public and cooperating agencies, vitally interested in the water resources of Kentucky, as to the current status of the Distfict's data collection and investigation program. Kentucky mines coal, limestone, clay, sand and gravel. Coal mining operations are carried out mainly in the Western Kentucky Coal Field and the Eastern Kentucky Coal field. As to nonfuel minerals, Mississippian limestones are mined in the Mississippian Plateaus Region and along Pine Mountain in southeastern Kentucky. Ordovician and Silurian limestones are mined from the central part of the state. Clay minerals that are mined in the state include common clay, ceramic and ball clays, refractory clay and shale. Following passage of the Patient Protection and Affordable Care Act (ACA) in the United States, the Kentucky Health Benefit Exchange, Kynect, began operating in Kentucky in October 2013. Kentucky expanded Medicaid eligibility in January 2014. Together, Kynect and Medicaid expansion provided access to affordable health care coverage to hundreds of thousands of individuals in Kentucky. However, following the Kentucky gubernatorial election in 2015, the newly inaugurated governor moved to dismantle Kynect and restructure the Medicaid expansion, jeopardizing public health gains and the state economy. As the first state to announce both the closure and restructuring of a state health insurance marketplace and Medicaid expansion, Kentucky may serve as a test case for the rest of the nation for reversal of ACA-related health policies. This article describes Kynect and the Kentucky Medicaid expansion and examines the potential short-term and long-term impacts that may occur following changes in state health policy. Furthermore, this article will offer potential strategies to ameliorate the expected negative impacts of disruption of both Kynect and the Medicaid expansion, such as the creation of a new state insurance marketplace under a new governor, the implementation of a private option, and increasing the state minimum wage for workers. Early school start times may curtail children's sleep and inadvertently promote sleep restriction. The current study examines the potential implications for early school start times for behavioral problems in public elementary schools (student ages 5-12 years) in Kentucky. School start times were obtained from school Web sites or by calling school offices; behavioral and disciplinary problems, along with demographic information about schools, were obtained from the Kentucky Department of Education. Estimated associations controlled for teacher/student ratio, racial composition, school rank, enrollment, and Appalachian location. Associations between early school start time and greater behavioral problems (harassment, in-school removals, suspensions, and expulsions) were observed, although some of these associations were found only for schools serving the non-Appalachian region. Findings support the growing body of research showing that early school start times may contribute to student problems, and extend this research through a large-scale examination of elementary schools, behavioral outcomes, and potential moderators of risk. Understanding Barriers to Colorectal Cancer Screening in Kentucky.

Colorectal cancer screening rates have increased significantly in Kentucky, from 35% in 1999 to 66% in 2012. A continued improvement in screening requires identification of existing barriers and implementation of interventions to address barriers. The state of Kentucky added a question to the 2012 Kentucky Behavioral Risk Factor Surveillance System survey for respondents aged 50 years or older who answered no to ever having been screened for colorectal cancer by colonoscopy or sigmoidoscopy to assess the reasons why respondents had not been screened. Combined responses constituted 4 categories: attitudes and beliefs, health care provider and health care systems barriers, cost, and other. Prevalence estimates for barriers were calculated by using raking weights and were stratified by race/ethnicity, sex, education, income, and health insurance coverage. Logistic regression estimated odds ratios for barriers to screening. The most common barriers in all areas were related to attitudes and beliefs, followed by health care provider and systems, and cost. Non-Hispanic whites and respondents with more than a high school education were more likely to choose attitudes and beliefs as a barrier than were non-Hispanic blacks and those with less than a high school education. Respondents with low incomes and with no insurance were significantly more likely to select cost as a barrier. No significant associations were observed between demographic variables and the selection of a health care provider and a health care system. In 1960, the U.S. Geological Survey and the Kentucky Geological Survey began a program to map the State geologically at a scale of 1:24,000 and to publish the maps as 707 U.S. Geological Survey Geologic Quadrangle Maps. Fieldwork was completed by the spring of 1977, and all maps were published by December 1978. Geologic mapping of the State was proposed by the Kentucky Society of Professional Engineers in 1959. Wallace W. Hagan, Director and State Geologist of the Kentucky Geological Survey, and Preston McGrain, Assistant State Geologist, promoted support for the proposal among organizations such as Chambers of Commerce, industrial associations, professional societies, and among members of the State government. It was also arranged for the U.S. Geological Survey to supply mapping personnel and to publish the maps; the cost would be shared equally by the two organizations. Members of the U.S. Geological Survey assigned to the program were organized as the Branch of Kentucky Geology. Branch headquarters, including an editorial staff, was at Lexington, Ky., but actual mapping was conducted from 18 field offices distributed throughout the State. The Publications Division of the U.S. Geological Survey established a cartographic office at Lexington to prepare the maps for publication. About 260 people, including more than 200 professionals, were assigned to the Branch of Kentucky Geology by the U.S. Geological Survey at one time or another. The most geologists assigned any one year was 61. To complete the mapping and ancillary studies, 661 professional man-years were required, compared with an original estimate of 600 man-years. A wide variety of field methods were used, but most geologists relied on the surveying altimeter to obtain elevations. Surface data were supplemented by drill-hole records, and several dozen shallow diamond-drill holes were drilled to aid the mapping. Geologists generally scribed their own maps, with a consequent saving of publication costs

Chronic pain is a prevalent chronic condition with significant burden and economic impact in the United States. Chronic pain is particularly abundant in primary care, with an estimated 52% of chronic pain patients obtaining care from primary care physicians (PCPs). However, PCPs often lack adequate training and have limited time and resources to effectively manage chronic pain. Chronic pain management is complex in nature because of high co-occurrence of psychiatric disorders and other medical comorbidities in patients. This article describes a quality improvement initiative conducted by the American College of Physicians (ACP), in collaboration with the Kentucky ACP Chapter, and the Center for Health Services and Outcomes Research at the Johns Hopkins Bloomberg School of Public Health, to enhance chronic pain management in 8 primary care practices participating in Accountable Care Organizations in Kentucky, with a goal of enhancing the screening, diagnosis, and treatment of patients with chronic pain.

The purpose of the study was to define genetic diversity of reptilian Salmonella enterica serovar (S.) Kentucky isolates and their epidemiological relations to the ones from poultry, food, and environmental origin in Poland. Between 2010 and 2012 twenty-four S. Kentucky isolates derived from snakes (N=8), geckos (N=7), chameleons (N=4), agamas (N=1), lizard (N=1), and environmental swabs taken from reptile exhibition (N=3) were identified. They were characterized with antimicrobial minimal inhibitory concentration testing, XbaI-PFGE and MLST typing. The profiles compared to S. Kentucky available in BioNumerics local laboratory database (N=40) showed 67.3% of relatedness among reptile isolates. Three genetic lineages were defined. The first lineage gathered 20 reptile isolates with 83.4% of similarity and wild-type MICs for all antimicrobials tested but streptomycin in single case. The remaining three reptilian and one post-exhibition environment S. Kentucky isolates were clustered (87.2%) with isolates originating from poultry, mainly turkey, food, and environment and presented variable non-wild type MICs to numerous antimicrobials. The third S. Kentucky lineage was composed of two isolates from feed (96.3%). The results suggest diverse sources and independent routes of infection. Most of the isolates belonged to reptile-associated clones spread both horizontally and vertically. Simultaneously, PFGE profiles and MLST type indistinguishable from the ones observed in poultry point out carnivore reptiles as possible vector of infection with multidrug and high-level ciprofloxacin resistant (MIC≥8 mg/L) S. Achieving 80% BSN by 2020: Lessons Learned From Kentucky's Registered Nurses.

The aim of this study was to understand the educational status and plans of Kentucky's RN workforce in advancing nursing educational levels. The Institute of Medicine called for 80% of nurses to hold a minimum of a BSN by 2020. Nurse leaders from practice, academe, and the community need evidence to guide the development of effective strategies. An electronic survey was administered to Kentucky's RNs. This descriptive analysis was based on 1363 usable responses. Only 40% of Kentucky's RNs held at least a BSN. Another 17% were enrolled in a nursing degree program; half of those enrolled were pursuing a BSN. Of those not enrolled in a degree program, 61.5% reported no plans to return to school. The top barriers were lack of perceived benefit, financial concerns, family obligations, and planned retirement. The top motivating factor was career advancement. The gap between the current reality and the goal is wide. Nurse leaders will need to develop creative strategies that strengthen motivating factors and reduce barriers to accelerate movement toward increasing BSN rates.

The purpose of this study was to investigate the association of selected characteristics of local health departments (LHDs) in Kentucky with the receipt of information by external stakeholders, specifically physicians and pharmacists, during the initial H1N1 outbreak of 2009. This study utilized a cross-sectional survey to gather characteristic information from local health departments. In addition, cross sectional surveys of physicians and pharmacists were used to determine information receipt. All 54 LHDs in Kentucky were surveyed; however, only those physicians belonging to the Kentucky Family Physician Association or the Kentucky Ambulatory Network were surveyed. Also, pharmacists included in this survey were members of the Kentucky Pharmacist Association. Descriptive data analyses, including chi-square test of independence, were conducted, and generalized estimating equations were used to calculate odds ratios to depict associations related to information exchange in this study. Response rates for the study were as follows: LHDs 65% (35/54), physicians 18.5% (96/518), and pharmacists 21.1% (211/1000). Of the 35 participating LHDs the most common characteristic identified was the presence of a public information officer (PIO) and a pandemic influenza plan, 76% and 64%, respectively. Despite these factors, 72% of external stakeholders did not receive any information regarding H1N1 from the LHD. Generalized estimating equations also indicated that stakeholders in jurisdictions lacking a PIO had 6 (95% confidence interval, 1.3-26.95) greater odds of not receiving information from the LHD. External stakeholders in jurisdictions without a pandemic influenza plan had 3.38 (95% confidence interval, 0.80-1.17) increased odds of not receiving information but this association was not statistically significant. Observations from this study indicate a need to improve information exchange between LHDs and their external stakeholders, specifically physicians and pharmacists

The geographical distribution of Ixodes scapularis and Amblyomma maculatum ticks is poorly understood in Kentucky. We conducted a convenience survey of wildlife species (white-tailed deer (Odocoileus virginianus), elk (Cervus canadensis) and black bears (Ursus americanus)) for ticks from October 2015 to January 2017. We detected four tick species including Amblyomma americanum, Dermacentor albipictus, I. scapularis and A. maculatum. Although the former two tick species were previously known to be widely distributed in Kentucky, we also found that I. scapularis and A. maculatum were also widespread. Because of the limited data available for pathogens from I. scapularis and A. maculatum, we tested them for Borrelia and Rickettsia spp. by polymerase chain reaction assays. Prevalence of Borrelia burgdorferi sensu stricto and Rickettsia parkeri were 11% and 3%, respectively. These data indicate that public health measures are important to prevent tick-borne diseases in Kentucky. Copyright © 2018 Elsevier GmbH. All rights reserved.

Safety status of farm tractors that operate on public highways in four rural Kentucky counties.

Kentucky FFA students inspected 153 farm tractors for safety features that prevent operator injuries during tractor overturns, highway collisions, runovers, and power take-off (PTO) entanglements. Tractor mean age was 23.6 years (SD = 20.9). Rollover protective structures (ROPS) were present on 50.66% of tractors, but only 33.33% of these had functional seatbelts. Loose and damaged seats were found on 30.46% of tractors. In 38.99% of cases, tractor rear-wheel fenders exposed operators to moving tractor tires, and 48.67% of tractors had dangerously worn or damaged tires. Tractors with a narrow front-end stance comprised 16.11% of the total. Only 53.06% of the tractors had starters with secure hard cover by-pass starting shields that fully covered the starter terminals, and 37.37% had fully exposed terminals with no cover. PTO master shields with all parts present and undamaged were present on only 29.27% of the tractors, and in 39.02% of cases the entire shield was missing. Only 44.67% of the tractors had properly mounted and fully functional mounting and dismounting access steps and handholds. SMV emblems were missing on 53.64% of tractors and in the proper place and condition in only 25.83% of cases. Tractors with properly mounted and fully functional head and tail lights comprised 40.94% of the sample, and tractors with no functional lights comprised 24.16%. Properly mounted, clean, and functional rearview mirrors were present on only 19.87% of the tractors, and 69.54% had no rearview mirrors. The project increased farming and non-farming students' awareness of tractor safety issues, provided empirical data about the safety status of a sample of tractors that frequently travel public highways in four rural Kentucky farming counties, and promoted dialog about these issues with adult farmers and other community members with whom the students interacted.

The Kentucky River basin, an area of approximately 7,000 sq mi, is divided into five hydrologic units that drain parts of three physiographic regions. Data on aquatic biological resources were collected and reviewed to assess conditions in the major streams for which data were available. The North, Middle, and South Forks of the Kentucky River are in the Eastern Coal Field physiographic region. Streams in this region are affected by drainage from coal mines and oil and gas operations, and many support only tolerant biotic stream forms. The Kentucky River from the confluence of the three forks to the Red River, is in the Knobs physiographic region. Oil and gas production operations and point discharges from municipalities have affected many streams in this region. The Red River, a Kentucky Wild River, supported a unique flora and fauna but accelerated sedimentation has eliminated many species of mussels. The Millers Creek drainage is affected by brines discharged from oil and gas operations, and some reaches support only halophilic algae and a few fish. The Kentucky River from the Red River to the Ohio River is in the Bluegrass physiographic region. Heavy sediment loads and sewage effluent from urban centers have limited the aquatic biota in this region. Silver Creek and South Elkhorn Creek have been particularly affected and aquatic communities in these streams are dominated by organisms tolerant of low dissolved oxygen concentrations. Biological data for other streams indicate that habitat and water quality conditions are favorable for most commonly occurring aquatic organisms. (USGS)

To compare the opinions of teachers in private and public schools about school nurses. This descriptive study was conducted at 10 randomly selected private and public schools in Izmir in western Turkey during April-May 2016, and comprised teachers who consented to participate. A questionnaire was used to collect the data about socio-demographic information of the subjects (six questions), problems frequently encountered in school and the teachers' opinions about school health nurses (seven questions). Two open-ended questions on the roles of school nurses and the most important health education topics were asked. SPSS 22 was used to analyse data. Of the 720 subjects, 517(72%) were females and 203(18%) were men. There were 360(50%) teachers from private schools and an equal number were from the public schools. The mean ages of the teachers from the private and public schools were 36.22±8.69 and 43.12±7.78 years, respectively. Besides, 337(93.6%) teachers of the private schools and 338(93.9%) teachers of the public school believed that school nurses were needed, while 19(5.3%) private school teachers and 162(45%) public school teachers said they had "no idea" about the roles of the school nurse. The most important role of the school nurse was promoting good health habits according to the public school teachers and health education according to the private school teachers. Half of the public school teachers said they had no idea about the role of a school nurse.

Although federal law requires all public schools to be smoke free, lack of compliance with the smoke-free policy is commonly reported. The aims of this study were to describe the indoor fine-particle (PM(2.5)) air pollution in a rural high school and surrounding public venues. This cross-sectional, nonexperimental study was conducted in Monroe County, Kentucky (population of 11,756). Fine-particle concentrations were measured in the high school and 5 public venues using spectrometers. Because of illegal student smoking, PM(2.5) concentrations were 19 times higher in the boys' student restroom than the National Ambient Air Quality Standard for outdoor air (670 vs. 35 microg/m(3)). The staff restrooms adjacent to the student restroom where staff did not smoke also showed high PM(2.5) levels. Average indoor air pollution in the public venues was 158 microg/m(3). Strict enforcement of smoke-free school policy and cessation resources are needed to reduce secondhand smoke exposure. Collaborative school-community campaigns involving parents, students, mass media, and community organizations may be effective in reducing the harm caused by tobacco. Implications for school nurses are discussed.

In response both to national pressures to reduce costs and improve health care access and outcomes and to local pressures to become a top-20 public research university, the University of Kentucky moved toward an integrated clinical enterprise, UK HealthCare, to create a common vision, shared goals, and an effective decision-making process. The leadership formed the vision and then embarked on a comprehensive and coordinated planning process that addressed financial, clinical, academic, and operational issues. The authors describe in depth the strategic planning process and specifically the definition of UK HealthCare's role in its medical marketplace. They began a rigorous process to assess and develop goals for the clinical programs and followed the progress of these programs through meetings driven by data and attended by the organization's senior leadership. They describe their approach to working with rural and community hospitals throughout central, eastern, and southern Kentucky to support the health care infrastructure of the state. They review the early successes of their strategic approach and describe the lessons they learned. The clinical successes have led to academic gains. The experience of UK HealthCare suggests that good business practices and good public policy are synergistic.

Smoke-free legislation and charitable gaming in Kentucky.

To determine the effect of municipal smoke-free laws in Kentucky on gross and/or net revenues from charitable gaming activities. Between January 2000 and June 2007, 13 Kentucky communities implemented smoke-free legislation; only three specifically exempted charitable gaming facilities and compliance in several communities was not consistent. Kentucky is a tobacco-growing state that has the highest smoking rate in the United States. A fixed-effects time series design to estimate the impact of municipal smoke-free laws on charitable gaming. 13 Kentucky counties that implemented smoke-free laws during the study period of January 2000 through June 2007. All charitable gaming facilities in 13 counties in which a smoke-free ordinance was enacted during the study period. Gross and net revenues from charitable gaming activities in each county for each quarter of the study period, obtained from the Kentucky Department of Charitable Gaming. When controlling for economic variables, county-specific effects and time trends using a robust statistical framework, there was no significant relation between smoke-free laws and charitable gaming revenues. Municipal smoke-free legislation had no effect on charitable gaming revenues. No significant harm to charitable gaming revenues was associated with the smoke-free legislation during the 7.5-year study period, despite the fact that Kentucky is a tobacco-producing state with higher-than-average smoking rates.

Dental caries among children visiting a mobile dental clinic in South Central Kentucky: a pooled cross-sectional study.

Dental caries is one of the most common chronic childhood diseases affecting a large portion of children in the United States. The prevalence of childhood dental caries in Kentucky is among the highest in the nation. The purposes of this study are to (1) compare sociodemographic differences between caries and no caries groups and (2) investigate factors associated with untreated dental caries among children who visited a mobile dental clinic in South Central Kentucky. Study subjects were children aged 6 to 15 years who participated in the school-based dental sealant program through the mobile dental clinic operated by the Institute for Rural Health at Western Kentucky University between September 2006 and May 2011 (n = 2,453). Descriptive statistics were calculated for sociodemographic factors (age, gender, race/ethnicity, insurance status, and urban versus rural residential location) and caries status. We used chi-square tests to compare sociodemographic differences of children stratified by caries and no caries status as well as three levels of caries severity. We developed a logistic regression model to investigate factors associated with untreated dental caries while controlling for sociodemographic characteristics. The proportion of children having untreated dental caries was 49.7% and the mean number of untreated dental caries was 2.0. The proportion of untreated dental caries was higher in older children, children with no insurance and living in rural residential locations, and caries severity was also higher in these groups. Odds ratio indicated that older ages, not having private insurance (having only public, government-sponsored insurance or no insurance at all) and rural residential location were associated with having untreated dental caries after controlling for sociodemographic characteristics of children. Digital flood-inundation maps for a 6.5-mile reach of Kentucky River at Frankfort, Kentucky, were created by the U.S. Geological Survey (USGS) in cooperation with the City of Frankfort Office of Emergency Management. The inundation maps, which can be accessed through the USGS Flood Inundation Mapping Science Web site at http://water.usgs.gov/osw/flood_inundation/, depict estimates of the areal extent and depth of flooding corresponding to selected water levels (stages) at the USGS streamgage Kentucky River at Lock 4 at Frankfort, Kentucky (station no. 03287500). Current conditions for the USGS streamgage may be obtained online at the USGS National Water Information System site (http://waterdata.usgs.gov/nwis/inventory?agency_code=USGS&site_no=03287500). In addition, the information has been provided to the National Weather Service (NWS) for incorporation into their Advanced Hydrologic Prediction Service (AHPS) flood warning system (http:/water.weather.gov/ahps/). The NWS forecasts flood hydrographs at many places that are often colocated at USGS streamgages. The forecasted peak-stage information, also available on the Internet, may be used in conjunction with the maps developed in this study to show predicted areas of flood inundation. In this study, flood profiles were computed for the Kentucky River reach by using HEC–RAS, a one-dimensional step-backwater model developed by the U.S. Army Corps of Engineers. The hydraulic model was calibrated by using the most current (2013) stage-discharge relation for the Kentucky River at Lock 4 at Frankfort, Kentucky, in combination with streamgage and high-water-mark measurements collected for a flood event in May 2010. Partners in Public Health: Public Health Collaborations With Schools of Pharmacy, 2015.

To collect data on public health collaborations with schools of pharmacy, we sent a short electronic survey to accredited and preaccredited pharmacy programs in 2015. We categorized public health collaborations as working or partnering with local and/or state public health departments, local and/or state public health organizations, academic schools or programs of public health, and other public health collaborations. Of 134 schools, 65 responded (49% response rate). Forty-six (71%) responding institutions indicated collaborations with local and/or state public health departments, 34 (52%) with schools or programs of public health, and 24 (37%) with local and/or state public health organizations. Common themes of collaborations included educational programs, community outreach, research, and teaching in areas such as tobacco control, emergency preparedness, chronic disease, drug abuse, immunizations, and medication therapy management. Interdisciplinary public health collaborations with schools of pharmacy provide additional resources for ensuring the health of communities and expose student pharmacists to opportunities to use their training and abilities to affect public health. Examples of these partnerships may stimulate additional ideas for possible collaborations between public health organizations and schools of pharmacy.

Objectives. Carbon monoxide (CO) poisoning is a leading cause of morbidity and mortality during natural disasters. On January 26–27, 2009, a severe ice storm occurred in Kentucky, causing widespread, extended power outages and disrupting transportation and communications. After the storm, CO poisonings were reported throughout the state. The objectives of this investigation were to determine the extent of the problem, identify sources of CO poisoning, characterize cases, make recommendations to reduce morbidity and mortality, and develop prevention strategies. Methods. We obtained data from the Kentucky Regional Poison Center (KRPC), hyperbaric oxygen treatment (HBOT) facilities, and coroners. Additionally, the Kentucky Department for Public Health provided statewide emergency department (ED) and hospitalization data. Results. During the two weeks after the storm, KRPC identified 144 cases of CO poisoning; exposure sources included kerosene heaters, generators, and propane heaters. Hospitals reported 202 ED visits and 26 admissions. Twenty-eight people received HBOT. Ten deaths were attributed to CO poisoning, eight of which were related to inappropriate generator location. Higher rates of CO poisoning were reported in areas with the most ice accumulation. Conclusions. Although CO poisonings are preventable, they continue to occur in postdisaster situations. Recommendations include encouraging use of CO alarms, exploring use of engineering controls on generators to decrease CO exposure, providing specific information regarding safe use and placement of CO-producing devices, and using multiple communication methods to reach people without electricity. Heavy rains over an extensive area on January 27-February 2, caused extreme flooding in southeastern Kentucky and adjacent areas in West Virginia, Virginia, and Tennessee. Total rainfall for the storm period ranged from 6-9 inches over most of the report area and was 12? inches at the eastern end of the Virginia-Kentucky State line. The principal basins affected by the storm were those of the Big Sandy, Kentucky, Cumberland, and Tennessee Rivers. Maximum discharge of record occurred in many streams. On Levisa Fork near Grundy, Va., the peak discharge of 33,200 cfs was 50 percent greater than the previous maximum in 17 years of record and was 3.3 times the mean annual flood. The peak discharges on-tributaries of the Kentucky River and on ,the Holston and Clinch Rivers were also the greatest of record and .those on the upper Cumberland River were nearly as great as .those during the historic floods of 1918 and 1946. Total flood damage was estimated at $61 million of which $39 million was in the Big Sandy River basin (mostly in Kentucky) and $15 million was in the Kentucky River basin--$52 million of the total damage was in Kentucky.

There is a need for a well-defined partnership between universities, and the business and industrial community to promote the transfer of technology. In an effort to foster such a partnership, the Space Systems Program, administered by NASA, has established information dissemination centers in cooperation with various universities throughout the country. As a result of limited success in the transfer of technology to state and local units of government NASA felt that new stimuli and new approaches were needed in the public sector area. NASA selected the University of Kentucky, a land grant institution with a significant research dissemination and service role, as the site for the new program. An annual report of this program at the University of Kentucky is presented.

The Kentucky Landscape Snapshot Project, a NASA-funded project, was established to provide a first baseline land cover/land use map for Kentucky. Through this endeavor, change detection will be institutionalized, thus aiding in decision-making at the local, state, and federal planning levels. 2002 Landsat 7 imaginery was classified following and Anderson Level III scheme, providing an enhancement over the 1992 USGS National Land Cover Data Set. Also as part of the deliverables, imperviousness and canopy closure layers were produced with the aid of IKONOS high resolution, multispectral imagery.

To compare medical, religious, and personal belief immunization exemption rates between private and public schools in US. Exemption rates were calculated using the Centers for Disease Control and Prevention School Immunization Assessment Surveys for the 2009-2010 school year excluding states with incomplete survey data. Standardized exemption rates weighted on enrollments in public and private schools were calculated. Differences in exemption rates between public and private schools were tested using Wilcoxon signed rank test. The overall state exemption rate was higher in US private than public schools, 4.25% (SD 4.27) vs 1.91% (1.67), P = .0001 and private schools had higher exemption rates for all types of exemptions; medical 0.58% (0.71) vs 0.34% (0.34) respectively (P = .0004), religious 2.09% (3.14) vs 0.83% (1.05) respectively (P = .0001), and personal belief 6.10% (4.12) vs 2.79% (1.57), respectively (P = .006). Overall exemption rates were significantly higher in states that allowed personal belief exemptions. Exemption rates were significantly higher in US private than in public schools. The clock is in the most southerly corner of the picture. Interstate 81 under construction extends northeast-southwest across the bottom portion of the photograph. The larger urban area nearest the center of the picture is Kingsport, Tennessee. On the southern side of I-80 and east of Kingsport is the city of Bristol, Tennessee-Virginia. Johnson City, Tennessee is the urban area near the edge of the picture southeast of Kingsport. The Holston River, a tributary of the Tennessee River, meanders through the Kingsport area. The characteristic ridge and valley features in the Cumberland Plateau of Kentucky, Tennessee and Virginia are clearly visible. Forests (dark green) occur on the ridges and clearly outline the folded and faulted rock formations. The valleys (light) were formed in the softer rocks as a result of erosion. Agricultural areas are indicated by the characteristic rectangular patterns. Coal production is an important industry of this area; and it is mined by surface open pit operations. The irregular light areas in the Kentucky-Virginia border area are the strip mines which follow the contour of the land. Reclamation of the strip mine areas is aided through accurate knowledge of the mine and drainage systems. The Impact of Emotional Intelligence on Conditions of Trust among Leaders at the Kentucky Department for Public Health

There has been limited leadership research on emotional intelligence and trust in governmental public health settings. The purpose of this study was to identify and seek to understand the relationship between trust and elements of emotional intelligence, including stress management, at the Kentucky Department for Public Health (KDPH). The KDPH serves as Kentucky’s state governmental health department. KDPH is led by a Commissioner and composed of seven primary divisions and 25 branches within those divisions. The study was a non-randomized cross-sectional study utilizing electronic surveys that evaluated conditions of trust among staff members and emotional intelligence among supervisors. Pearson correlation coefficients and corresponding p-values are presented to provide the association between emotional intelligence scales and the conditions of trust. Significant positive correlations were observed between supervisors’ stress management and the staff members’ trust or perception of supervisors’ loyalty (r = 0.6, p = 0.01), integrity (r = 0.5, p = 0.03), receptivity (r = 0.6, p = 0.02), promise fulfillment (r = 0.6, p = 0.02), and availability (r = 0.5, p = 0.07). This research lays the foundation for emotional intelligence and trust research and leadership training in other governmental public health settings, such as local, other state, national, or international organizations. This original research provides metrics to assess the public health workforce with attention to organizational management and leadership constructs. The survey tools could be used in other governmental public health settings in order to develop tailored training opportunities related to emotional intelligence and trust organizations. Regionalization of harmonic-mean streamflows in Kentucky

Harmonic-mean streamflow (Qh), defined as the reciprocal of the arithmetic mean of the reciprocal daily streamflow values, was determined for selected stream sites in Kentucky. Daily mean discharges for the available period of record through the 1989 water year at 230 continuous record streamflow-gaging stations located in and adjacent to Kentucky were used in the analysis. Periods of record affected by regulation were identified and analyzed separately from periods of record unaffected by regulation. Record-extension procedures were applied to short-term stations to reducetime-sampling error and, thus, improve estimates of the long-term Qh. Techniques to estimate the Qh at ungaged stream sites in Kentucky were developed. A regression model relating Qh to total drainage area and streamflow-variability index was presented with example applications. The regression model has a standard error of estimate of 76 percent and a standard error of prediction of 78 percent.

Bringing Science Public Outreach to Elementary Schools

Many science "museums” already offer fantastic programs for the general public, and even some aimed at elementary school kids. However, these venues are usually located in large cities and are only occasionally used as tools for enriching science education in public schools. Here we present preliminary work to establish exciting educational enrichment environments for public schools that do not easily have access to such facilities. This program is aimed at motivating children's interest in science beyond what they learn in the classroom setting. In this program, we use the experience and experiments/demonstrations developed at a large science museum (in this case, The St. Louis Science Center) and take them into a local elementary school. At the same time, students from the University of Missouri are getting trained on how to present these outreach materials and work with the local elementary schools. Our pilot study has started with implementation of presentations/demonstrations at Benton Elementary School within the Columbia Public School district, Missouri. The school has recently adopted a STEM (Science, Technology, Engineering, and Mathematics) centered learning system throughout all grade levels (K-5), and is therefore receptive to this effort. We have implemented a program in which we have given a series of scientific demonstrations at each grade level's lunch hour. Further enrichment ideas and plans include: addition demonstrations, hands-on experiments, and question and answer sessions. However, the application of these events would be to compliment the curriculum for the appropriate grade level at that time. The focus of this project is to develop public communications which links science museums, college students and local public schools with an emphasis on encouraging college science majors to share their knowledge and to strengthen their ability to work in a public environment.

This review of the lignite deposits of western Tennessee and the Jackson Purchase area in western Kentucky (Figure 1) is an updated report on part of the U.S. Geological Survey's National Coal Resource Assessment of the Gulf Coastal Plain Coal Province (see Ruppert et al., 2002; Hackley et al., 2006; Dennen, 2009; and other chapters of this publication). Lignite deposits of western Kentucky and Tennessee are an extension of the Gulf Coastal Plain Coal Province (Cushing et al., 1964), and currently are not economic to mine. These deposits have not been extensively investigated or developed as an energy resource. This review includes a description of the geology of the lignite-bearing units, a discussion of the available coal quality data, and information on organic petrology. Palynological data for lignite samples collected in Kentucky and Tennessee as part of this work are presented in Table 1. Lignite trace element data originally presented in Hackley et al. (2006) are not included in this report due to potential laboratory quality control issues during the time the samples were analyzed (U.S. Geological Survey Energy Resources Program, 2010).

 The presence of athletic trainers (ATs) in secondary schools to provide medical care is crucial, especially with the rise in sports participation and resulting high volume of injuries. Previous authors have investigated the level of AT services offered, but the differences in medical care offered between the public and private sectors have not been explored.  To compare the level of AT services in public and private secondary schools.  Concurrent mixed-methods study.  Public and private secondary schools in the United States.  A total of 10 553 secondary schools responded to the survey (8509 public, 2044 private).  School administrators responded to the survey via telephone or e-mail. Descriptive statistics depict national data. Open-ended questions were evaluated through content analysis.  A greater percentage of public secondary schools than private secondary schools hired ATs. Public secondary schools provided a higher percentage of full-time, part-time, and clinic AT services than private secondary schools. Only per diem AT services were more frequent in the private sector. Regardless of the extent of services, reasons for not employing an AT were similar between sectors. Common barriers were budget, school size, and lack of awareness of the role of an AT. Unique to the public sector, remote location was identified as a challenge faced by some administrators.  Both public and private secondary schools lacked ATs, but higher percentages of total AT services and full-time services were available in the public sector. Despite differences in AT services, both settings provided a similar number of student-athletes with access to medical care. Barriers to hiring ATs were comparable between public and private secondary schools; however, remote location was a unique challenge for the public sector.

HB 238 has been reported favorably to the floor of the House of Representatives by the Committee on Licensing and Occupations, but has not yet been acted upon by the full House. When asked whether he thought his legislation would be approved, Representative Burch said he thought the bill had a "50-50" chance of passing the Kentucky legislature this year. Sources within the Kentucky dental laboratory and dental communities disagree on the legislation's chances for passage. Some within the industry feel the legislation simply does not have enough votes to pass the General Assembly, primarily because of opposition from the Kentucky Dental Association. Others speculate that the bill has a better opportunity to be approved this year than it did in the previous years when it was introduced. The Kentucky General Assembly adjourns its session for the year 2000 on April 12, so there will be a quick answer to the question of whether HB 238 will be approved. The May issue of this column will provide details.

Public schools provide students with opportunities to participate in many discretionary, unmandated wellness programs. Little is known about the number of these programs, their distribution across schools, and the kinds of students served. We provide evidence on these questions for New York City (NYC) public schools. Data on wellness programs were collected from program websites, NYC's Office of School Food and Wellness, and direct contact with program sponsors for 2013. Programs were grouped into categories, nutrition, fitness, and comprehensive, and were combined with data on school characteristics available from NYC's Department of Education. Numbers of programs and provision of programs were analyzed for relationships with demographic and school structural characteristics, using descriptive statistics and multiple regression. Discretionary wellness programs are numerous, at 18 programs. Little evidence supports inequity according to student race/ethnicity, income, or nativity, but high schools, new schools, co-located schools, small schools, and schools with larger proportions of inexperienced teachers are less likely to provide wellness programs. Opportunities exist to further the reach of wellness programs in public schools by modifying them for high school adoption and building capacity in schools less likely to have the administrative support to house them. © 2016, American School Health Association.

To describe the percentage of US public health schools and programs offering graduate-level courses with disability content as a potential baseline measurement for Healthy People 2020 objective DH-3 and compare the percentage of public health schools that offered disability coursework in 1999 with those in 2011. In 2011, using SurveyMonkey.com, cross-sectional information was collected from the deans, associate deans, directors, or chairpersons of master of public health-granting public health schools and programs that were accredited and listed with the Council on Education for Public Health. Two rounds of follow-up were conducted at 4-month intervals by e-mails and phone calls to program contacts who had not responded. The responses from schools and programs were calculated and compared. There were 78 responses (34 schools and 44 programs) for a response rate of 63%. Fifty percent of public health schools and programs offered some disability content within their graduate-level courses. A greater percentage of schools than programs (71% vs 34%; P = .003) offered some graduate-level disability coursework within their curricula. The percentage of schools that offered disability coursework was similar in 1999 and 2011. This assessment provides a potential baseline measurement for Healthy People 2020 objective DH-3. Future assessments should focus on clarifying disability content within courses and identifying capacity to offering disability training within public health schools and programs.

Droughts of the twentieth and early twenty-first centuries: Influences on the production of beef and forage in Kentucky, USA.

Drought affects societies world-wide in many different ways. It is a natural hazard that is complex and not well understood and as a result, its impacts are often poorly documented. The purpose of this research is to quantify (in dollars) the impacts of drought on Kentucky's beef and forage (hay) production. Observations suggest that the most important droughts in Kentucky occurred in 1930-31, 1940-42, 1952-55, 1987-88, 1999-2000 and 2007. The total state revenue for these commodities were analyzed during these severe drought years and non-drought years. The research estimated revenue deficit from these severe droughts in Kentucky for these (beef and hay) agricultural commodities. This study is important to the general public as well as planners and policy makers. A public health training center experience: professional continuing education at schools of public health.

The Public Health Training Center (PHTC) national program was first established at accredited schools of public health in 2000. The PHTC program used the US Health Resources and Services Administration's grants to build workforce development programs, attracting schools as training providers and the workforce as training clients. This article is a reflection on the experience of two schools, whose partnership supported one of the PHTCs, for the purpose of opening a conversation about the future of continuing education throughout schools and degree programs of public health. This partnership, the Pennsylvania & Ohio Public Health Training Center (POPHTC), concentrated its funding on more intensive training of public healthcare workers through a relatively narrow inventory of courses that were delivered typically in-person rather than by distance-learning technologies. This approach responded to the assessed needs and preferences of the POPHTC's workforce population. POPHTC's experience may not be typical among the PHTCs nationally, but the collective experience of all PHTCs is instructive to schools of public health as they work to meet an increasing demand for continuing education from the public health workforce.

...In this notice, EPA is notifying the public that EPA has made an insignificance finding through the transportation conformity adequacy process for directly emitted fine particulate matter (PM2.5) and nitrogen oxides (NOX) emissions as contained in the 1997 PM2.5 attainment demonstration for the Kentucky portion of the tri-state Huntington-Ashland nonattainment area. The tri-state Huntington-Ashland 1997 annual PM2.5 nonattainment area is comprised of Boyd County, Kentucky; Cabell and Wayne Counties, West Virginia; Lawrence and Scioto Counties, Ohio; and the partial Counties of Lawrence in Kentucky; Mason in West Virginia; and Adams and Gallia in Ohio. On December 5, 2008, the Commonwealth of Kentucky, through the Division of Air Quality (DAQ), submitted an attainment demonstration for the 1997 annual PM2.5 standard for the Kentucky portion of the tri-state Huntington-Ashland area (hereafter referred to as the Ashland Area). As a result of EPA's insignificance finding, the Ashland Area is no longer required to perform regional emissions analyses for either directly emitted PM2.5 or NOX as part of future PM2.5 conformity determinations for the 1997 annual PM2.5 air quality standard. This finding only relates to the Kentucky portion of the tri-state Huntington-Ashland 1997 PM2.5 nonattainment area. In a letter dated October 23, 2009, EPA informed the State of Ohio that regional mobile emissions of direct PM2.5 and NOX are insignificant for transportation conformity purposes as well. That insignificance finding took effect on December 22, 2009. EPA will review the adequacy of the West Virginia submittal in a separate action.

Health Education Through Extension Leaders (HEEL) is one of the solutions the University of Kentucky College of Agriculture has created to address the problem of chronic disease in Kentucky. Building on the land grant model for education, outreach, and prevention, HEEL collaborates and partners with the academic health centers, area health education centers, the Center for Rural Health, the Kentucky Cancer Program, the Markey Cancer Center, the University of Kansas Wellness Program, and the Kentucky Cabinet for Health and Family Services to implement research-based preventive programs to the county extension agents across Kentucky. Extension agents are an instrumental bridge between the communities across Kentucky and the educational resources provided by the HEEL program.

This study describes and analyzes school nurses' (SN) experiences with understanding and using public health interventions from the Public Health Intervention Wheel. The Wheel offers a model for naming interventions provided by SNs from a public health perspective. Research teams from academic and SN practice settings conducted six focus groups with school nurses from Minnesota. Participants were asked to share experiences through telling stories from their practice that represented a specific wedge of the Wheel. Researchers organized data by intervention; often stories represented more than one intervention. Stories represented all levels of practice. This study highlights important contributions of school nurses to promote the health of school populations through the use of Wheel interventions. The integration of Wheel interventions in the application of the Framework for 21st-Century School Nursing Practice™ provides SNs with a language to document and communicate their expert professional practice.

Water-use information for 1990 was collected and reported, by county, for eight major categories of use. Seven of the categories were offstream uses, which included public supply, commercial, domestic, industrial, mining, thermoelectric, and agricultural uses. The agricultural category was subdivided into irrigation and livestock water use. Instream water- use data also were collected for hydroelectric-power generation. Estimated average water use in Kentucky exceeded 4,300 million gallons per day during 1990 for all offstream uses. About 94 percent of this amount was from surface-water sources, and about 6 percent was from ground-water sources. Per capita use for all offstream uses was almost 1,200 gallons per day. Estimated average consumptive use was 309 million gallons per day. Estimated average instream water use for hydroelectric-power generation was 83,000 million gallons per day. Ninety-seven percent of the offstream water withdrawals during 1990 were withdrawn for thermoelectric, public supply, and industrial use. Cooling water used in the production of thermoelectric power accounted for about 80 percent of the total offstream water use during 1990. Water withdrawn for public supplies was second largest at almost 10 percent of the total, and industrial water withdrawals were about 7 percent of the total. Thermoelectric, domestic, and livestock uses accounted for almost 90 percent of the consumptive use during 1990. The thermoelectric category accounted for almost two-thirds of the total consumptive use in the State for all uses.

This study investigated the EFL reading goals of Grade 11 students across public and non-public schools in the Ethiopian capital, Addis Ababa. To this end, quantitative data were collected from 556 (375 public and 181 non-public) students via pre-tested structured questionnaire and analyzed into means, medians, standard deviations, ranges and Mann-Whitney U test scores. The results show that non-public school students were found better than public school students in possessing components of both extrinsic and intrinsic goals for reading. The notable exception in this regard is that public school students had higher social motivation for reading than their non-public school counterparts. Based on this finding, it has been concluded that non-public school students have a better chance of evolving as persistent self-initiated EFL readers since they have various goals which urge them to engage in reading a range of texts. It is thus recommended that English language teachers in public schools should constantly take actions to enable their students to develop appropriate EFL reading goals.

The report reviews and summarizes 10,578 chemical analyses, from 2,362 wells and springs in Kentucky. These water-quality data were collected prior to September 30, 1981, and are available in computer files of the U.S. Geological Survey. The principal water-bearing rocks in Kentucky were combined into 10 major groups to aid in data summary preparation and general description of the ground-water quality of the State. Ground water in Kentucky is generally fresh near the outcrop of the rocks comprising the aquifer. Slightly saline to briny water occurs at variable depths beneath the freshwater. Preparation of quadrilinear diagrams revealed three principal geochemical processes in the aquifers of Kentucky: (1) mixing of freshwater and saline water in an interface zone; (2) dedolomitization of the Devonian and Silurian and Lower Mississippian carbonate rocks; (3) sodium for calcium exchange in the freshwater sections of many of the sandstone-shale aquifers. A number of errors and deficiencies were found in the data base. The principal deficiencies were: (1) very few complete analyses which included important field measurements; (2) inadequate definition of the chemistry of the freshwater-saline water interface zone throughout much of the State; (3) no analyses of stable isotopes and dissolved gases; (4) fewer than 10 analyses of most trace metals, radionuclides, and man-made organic chemicals; and (5) no data on bacteria in ground water from any aquifer in the State. Educating the future public health workforce: do schools of public health teach students about the private sector?

Recent surveys indicate that approximately 40% of graduates from schools of public health are employed within the private sector or have an employer charged with regulating the private sector. These data suggest that schools of public health should provide curricular opportunities for their students--the future public health workforce--to learn about the relationship between the private sector and the public's health. To identify opportunities for graduate students in schools of public health to select course work that educates them about the relationship between the private sector and public health. We systematically identified and analyzed data gathered from publicly available course titles and descriptions on the Web sites of accredited schools of public health. Data were collected in the United States. The sample consisted of accredited schools of public health. Descriptions of the number and types of courses that schools of public health offer about the private sector and identification of how course descriptions frame the private sector relative to public health. We identified 104 unique courses with content about the private sector's relationship to public health. More than 75% of accredited schools of public health offered at least 1 such course. Nearly 25% of identified courses focused exclusively on the health insurance industry. Qualitative analysis of the data revealed 5 frames used to describe the private sector, including its role as a stakeholder in the policy process. Schools of public health face a curricular gap, with relatively few course offerings that teach students about the relationship between the private sector and the public's health. By developing new courses or revising existing ones, schools of public health can expose the future public health workforce to the varied ways public health professionals interact with the private sector, and potentially influence students' career paths.

OBJECTIVE--To investigate the frequency and severity of accidents to children in schools. METHODS--Retrospective analysis of accidents occurring in school over a six month period and resulting in attendance at an accident and emergency department. Comparison was made with accidents occurring in public places, RESULTS--During the study period there were 127 school and 251 public place accidents causing fractures or dislocations requiring hospital treatment in 3-16 year old children; 22.4% (127/567) of all injuries at school resulted in fractures or dislocations, as opposed to 20.8% (251/1197) of those sustained in public places. CONCLUSIONS--Injuries at school are a cause for concern as they occur in a supervised environment and this should be the target of accident prevention measures. PMID:8733659

Variation in exemptions to school immunization requirements among New York State private and public schools.

School immunization requirements have ensured high vaccination rates and have helped to control vaccine-preventable diseases. However, vaccine exemptions have increased in the last decade. This study compared New York State private versus public schools with respect to medical and religious exemption rates. This retrospective study utilizes New York State Department of Health Immunization Survey data from the 2003 through 2012 academic years. Schools were categorized as private or public, the former further categorized by religious affiliation. Rates of medical and religious vaccine exemptions were compared by school category. From 2003 to 2012, religious exemptions increased in private and public schools from 0.63% to 1.35% and 0.17% to 0.29% (Spearman's R: 0.89 and 0.81), respectively. Among private schools, increases in religious exemption rates during the study period were observed in Catholic/Eastern Orthodox, Protestant/Other Christian, Jewish, and secular schools (Spearman's R=0.66, 0.99, 0.89, and 0.93), respectively. Exemption rate ratios in private schools compared to public schools were 1.39 (95% CI 1.15-1.68) for medical and 3.94 (95% CI: 3.20-4.86) for religious exemptions. Among private school students, all school types except for Catholic/Eastern Orthodox and Episcopal affiliates were more likely to report religious exemptions compared to children in public schools. Medical and religious exemption rates increased over time and higher rates were observed among New York State private schools compared to public schools. ...EPA is taking final action to approve the December 13, 2007, submission by the Commonwealth of Kentucky, through the Kentucky Division of Air Quality (KDAQ) as demonstrating that the Commonwealth meets the state implementation plan (SIP) requirements of sections 110(a)(1) and (2) of the Clean Air Act (CAA or the Act) for the 1997 8- hour ozone national ambient air quality standards (NAAQS). Section 110(a) of the CAA requires that each state adopt and submit a SIP for the implementation, maintenance, and enforcement of each NAAQS promulgated by the EPA, which is commonly referred to as an ``infrastructure'' SIP. Kentucky certified that the Kentucky SIP contains provisions that ensure the 1997 8-hour ozone NAAQS is implemented, enforced, and maintained in Kentucky (hereafter referred to as ``infrastructure submission''). Kentucky's infrastructure submission, provided to EPA on December 13, 2007, addressed all the required infrastructure elements for the 1997 8-hour ozone NAAQS. Additionally, EPA is responding to adverse comments received on EPA's March 17, 2011, proposed approval of Kentucky's December 13, 2007, infrastructure submission.

We have been looking at two different numbers that have been used to describe the availability of physics in U.S. high schools: 60% and 95%. Last month we noted that the U.S. Department of Education Office of Civil Rights (OCR) includes over 7000 more public schools in the denominator than AIP Statistics does. Almost 4000 of the additional schools are special education, alternative, or other schools that do have seniors enrolled. An additional ˜3350 are schools that have a grade 9, 10, 11, or 12, but no seniors enrolled. The 95% number reported by AIP Statistics is the proportion of seniors attending a school where physics is offered regularly. The italicized words explain the bulk of the difference between the 60% and 95%.

Divergent evolution in fluviokarst landscapes of central Kentucky

Central Kentucky is characterized by a mixture of karst and fluvial features, typically manifested as mosaic of karst-rich/ channel-poor (KRCP) and channel-rich/karst-poor (CRKP) environments. At the regional scale the location and distribution of KRCP and CRKP areas are not always systematically related to structural, lithological, topographic, or other controls. This study examines the relationship of KRCP and CRKP zones along the Kentucky River gorge area, where rapid incision in the last 1??5 million years has lowered local base levels and modified slopes on the edge of the inner bluegrass plateau. At the scale of detailed field mapping on foot within a 4 km2 area, the development of karst and fluvial features is controlled by highly localized structural and topographic constraints, and can be related to slope changes associated with retreat of the Kentucky River gorge escarpment. A conceptual model of karst/fluvial transitions is presented, which suggests that minor, localized variations are sufficient to trigger a karst-fluvial or fluvial-karst switch when critical slope thresholds are crossed. ?? 2004 John Wiley and Sons, Ltd.

Farm work injuries among a cohort of children in Kentucky, USA.

Children residing on farms with livestock may be at an increased risk for work-related injuries, compared to children who work on other commodity farms. This study characterizes children's work tasks on Kentucky farms and assesses whether children who work on beef cattle farms are at an increased risk for farm work injuries. The results of a cohort study of children aged 5-18 years (N=999 at baseline) working on family farms in Kentucky, followed for two consecutive years after an initial enumeration five years previously, found that 70% of the children were involved in animal-related chores. Across all age groups, children on beef cattle farms devoted a greater number of hours per week to farm work, compared to children living on other commodity farms, especially during the school year. For all children in the study, working more than 180 days per year, performing farm work independently, and working on a beef cattle farm (compared to other commodity farm), increased the risk of a farm work injury. However, none of these associations were statistically significant. For male children only, the performance of work tasks independently was significantly associated with a 2.4-fold increased risk (OR = 2.41; 95% CI: 1.15-5.06; P=0.02) for a farm work injury, after controlling for days of working, age, period of data collection, and commodity type of the farm.

Meaningful Use and Electronic Laboratory Reporting: Challenges Health Information Technology Vendors Face in Kentucky.

To explore the challenges Health Information Technology (HIT) vendors face to satisfy the requirements for Meaningful Use (MU) and Electronic Laboratory Reporting (ELR) of reportable diseases to the public health departments in Kentucky. A survey was conducted of Health Information Exchange (HIE) vendors in Kentucky through the Kentucky Health Information Exchange (KHIE). The survey was cross-sectional. Data were collected between February and March 2014. Participants were recruited from KHIE vendors. Participants received online survey link and by email and asked to submit their responses. Vendors' feedback were summarized and analyzed to identify their challenges. Out of the 55 vendors who received the survey, 35(63.64%) responded. Of the seven transport protocol options for ELR, vendors selected virtual private network (VPN) as the most difficult to implement (31.7%). Secure File Transfer Protocol (SFTP) was selected as preferred ELR transport protocol (31.4%). Most of the respondents, 80% responded that they do not have any challenge with the Health Level 7 (HL7) standard implementation guide required by MU for 2014 ELR certification. The study found that the most difficult transport protocol to implement for ELR is VPN and if vendors have preference, they would use SFTP for ELR over KHIE choice of VPN and Simple Object Access Protocol (SOAP). KHIE vendors do not see any variability in what is reportable by different jurisdiction and also it is not difficult for them to detect what is reportable from one jurisdiction verse the other.

Water use in Kentucky, 1985

Water use information for 1985 was collected and reported by county for eight major categories of use. Seven of the categories were offstream uses, which included (1) public supply , (2) commercial, (3) domestic, (4) industrial, (5) mining, (6) thermoelectric, and (7) agricultural uses. The agricultural uses category was separated into irrigation and livestock water use. Instream water-use data also were collected for hydroelectric power generation. Water use in Kentucky was estimated to average 4,200 million gallons per day (M gal/d) during 1985 for all offstream uses. About 95% of this amount was from surface water sources, and about 5% was from groundwater sources. Per capita use for all offstream uses was about 1,100 gal/day. About 260 M gal/d was consumed (not returned to the surface--or groundwater system). Instream water use for hydroelectric power generation was estimated to be 91,000 M gal/d. Almost 97% of the offstream water withdrawals in 1985 were withdrawn for thermoelectric, public supply, and industrial use. Cooling water used in the production of thermoelectric power accounted for about 81% of the total offstream water use in 1985. Water withdrawn for public supplies was the second largest offstream use at about 10 % of the total, and industrial water use was almost 6%. Thermoelectric, domestic, and livestock uses accounted for almost 90% of the consumptive use in 1985. Consumptive use in the thermoelectric category was almost one-half of the total amount consumed in the State for all uses. (USGS)
