The objective of this article is to introduce the reader to this special supplement to the Journal of Occupational and Environmental Medicine regarding Federal Workers' Compensation Programs. The short history of both the VHA and DoD Federal Workers' Compensation Programs are provided and a short synopsis of each author's article is provided. The lessons learned from the articles in the supplement are summarized in this article and 6 key findings are highlighted. Cooperation between human resources workers' compensation personnel, safety and occupational health personnel is a must for successful management of the WC program. Information and data sharing are critical for root cause and injury prevention, case management, and cost containment efforts. Enhancing efforts in these areas will save an estimated $100 million through cost avoidance efforts.

The U.S. Office of Personnel Management (OPM) is issuing an interim final regulation to update the Federal Employees Health Benefits Program (FEHBP) and the Federal Employees Dental and Vision Insurance Program (FEDVIP) regulations to reflect updated election opportunities for participants in the Pathways Programs. The Pathways Programs were created by Executive Order (E.O.) 13562, signed by the President on December 27, 2010, and are designed to enable the Federal Government to compete effectively for students and recent graduates by improving its recruitment efforts through internships and similar programs with Federal agencies. This interim final rule furthers these recruitment and retention efforts by providing health insurance, as well as dental and vision benefits, to eligible program participants and their families.

Midwives are working in federally funded health centers in increasing numbers. Health centers provide primary and preventive health care to almost 20 million people and are located in every US state and territory. While health centers serve the entire community, they also serve as a safety net for low-income and uninsured individuals. In 2010, 93% of health center patients had incomes below 200% of the Federal Poverty Guidelines, and 38% were uninsured. Health centers, including community health centers, migrant health centers, health care for the homeless programs, and public housing primary care programs, receive grant funding and enjoy other benefits due to status as federal grantees and designation as federally qualified health centers. Clinicians working in health centers are also eligible for financial and professional benefits because of their willingness to serve vulnerable populations and work in underserved areas. Midwives, midwifery students, and faculty working in, or interacting with, health centers need to be aware of the regulations that health centers must comply with in order to qualify for and maintain federal funding. This article provides an overview of health center regulations and policies affecting midwives, including health center program requirements, scope of project policy, provider credentialing and privileging, Federal Tort Claims Act malpractice coverage, the 340B Drug Pricing Program, and National Health Service Corps scholarship and loan repayment programs. © 2012 by the American College of Nurse-Midwives.

The federal sector is the largest consumer of products in the United States and annually purchases almost 1.5 quads of energy measured at the building site at a cost of almost $10 billion (U.S. Department of Energy 1991). A review of design, construction, and procurement practices in the federal sector, as well as discussions with manufacturers and vendors, indicated that new technologies are not utilized in as timely a manner as possible. As a consequence of this technology transfer lag, the federal sector loses valuable energy and environmental benefits that can be derived through the application of new technologies. Inmore » addition, opportunities are lost to reduce federal energy expenditures and spur U.S. economic growth through the procurement of such technologies. In 1990, under the direction of the U.S. Department of Energy (DOE) Federal Energy Management Program, the Pacific Northwest Laboratory began the design of a program to accelerate the introduction of new U.S. technologies into the federal sector. Designated first as the Test Bed Demonstration Program and more recently the New Technology Demonstration Program, it sought to shorten the acceptance period of new technologies within the federal sector. The Federal Employees Health Benefits Program (FEHBP) can serve as a vehicle to extend access to private health insurance to nonfederal workers. Opening up the FEHBP may also be a politically feasible incremental change because it potentially appeals to those at both ends of the political spectrum. Each of the various ways of enabling nonfederal workers to participate in the FEHBP has its pros and cons. Ecosystem-based management (EBM) is an integrated management approach that considers the entire ecosystem, including humans, across multiple sectors, with the goal to collectively manage natural resources, habitat, and species in a sustainable manner while maintaining ecosystem services to humans on the long-term. In the United States, the National Ocean Council (NOC) established a federal interagency subgroup (National Ocean Policy EBM-Subgroup) to provide policy advice on EBM strategies and technical representation from the federal agencies that are part of the NOC. As part of the NOP EBM-Subgroup effort, this study summarizes the status of EBM for several federal programs within the NOC agencies that implement or support marine and coastal EBM activities. Our objective was to provide an overview of the current state of practice among the many and varied U.S. federal programs employing EBM approaches in the ocean, coastal zone, and the Great Lakes. We used social network analysis techniques to explore similarities among programs in different topic areas (e.g., type of audience, partners, training, EBM best management practices and principles). Results highlight substantial differences in perceived and effective performances across programs, with Management programs showing a higher level of integration of EBM approaches than Non-Management programs. The use of EBM best management practices and principles among programs is unbalanced, with some key elements of EBM strategies less commonly employed in the management planning. This analysis identified gaps in the implementation of EBM strategies that can inform natural resource managers and planners

The Nation's economic prosperity and military security depend heavily on development and commercialization of advanced materials. Materials are a key facet of many technologies, providing the key ingredient for entire industries and tens of millions of jobs. With foreign competition in many areas of technology growing, improvements in materials and associated processes are needed now more than ever, both to create the new products and jobs of the future and to ensure that U.S. industry and military forces can compete and win in the international arena. The Federal Government has invested in materials research and development (R&D) for nearly amore » century, helping to lay the foundation for many of the best commercial products and military components used today. But while the United States has led the world in the science and development of advanced materials, it often has lagged in commercializing them. This long-standing hurdle must be overcome now if the nation is to maintain its leadership in materials R&D and the many technologies that depend on it. The Administration therefore seeks to foster commercialization of state-of-the-art materials for both commercial and military use, as a means of promoting US industrial competitiveness as well as the procurement of advanced military and space systems and other products at affordable costs. The Federal R&D effort in Fiscal Year 1994 for materials science and technology is an estimated $2123.7 million. It includes the ongoing R&D base that support the missions of nine Federal departments and agencies, increased strategic investment to overcome obstacles to commercialization of advanced materials technologies, interagency cooperation in R&D areas of mutual benefit to leverage assets and eliminate duplicative work, cost-shared research with industrial and academic partners in critical precompetitive technology areas, and international cooperation on selected R&D topics with assured benefits for the United States

OSHA is issuing a final rule amending the Basic Program Elements to require Federal agencies to submit their occupational injury and illness recordkeeping information to the Bureau of Labor Statistics (BLS) and OSHA on an annual basis. The information, which is already required to be created and maintained by Federal agencies, will be used by BLS to aggregate injury and illness information throughout the Federal government. OSHA will use the information to identify Federal establishments with high incidence rates for targeted inspection, and assist in determining the most effective safety and health training for Federal employees. The final rule also interprets several existing basic program elements in our regulations to clarify requirements applicable to Federal agencies, amends the date when Federal agencies must submit to the Secretary of Labor their annual report on occupational safety and health programs, amends the date when the Secretary of Labor must submit to the President the annual report on Federal agency safety and health, and clarifies that Federal agencies must include uncompensated volunteers when reporting and recording occupational injuries and illnesses.

Federally Funded Programs Related to Building Energy Use: Overlaps, Challenges, and Opportunities for Collaboration

As energy efficiency in buildings continues to move from discreet technology development to an integrated systems approach, the need to understand and integrate complementary goals and targets becomes more pronounced. Whether within Department of Energy’s (DOE) Building Technologies Program (BTP), across the Office of Energy Efficiency and Renewable Energy (EERE), or throughout DOE and the Federal government, mutual gains and collaboration synergies exist that are not easily achieved because of organizational and time constraints. There also cases where federal agencies may be addressing similar issues, but with different (and sometimes conflicting) outcomes in mind. This report conducts a comprehensive inventorymore » across all EERE and other relevant Federal agencies of potential activities with synergistic benefits. A taxonomy of activities with potential interdependencies is presented. The report identifies a number of federal program objectives, products, and plans related to building energy efficiency and characterizes the current structure and interactions related to these plans and programs. Areas where overlap occurs are identified as are the challenges of addressing issues related to overlapping goals and programs. Using the 2005 National Association of County and City Health Officers Profile of Local Health Departments data set, bivariate probit and Heckman selection models were used to test the hypothesis that the level of federal funding received for bioterrorism preparedness is related to the preparedness activities undertaken by local health departments. Overall budget, leadership, and crisis experience are found to be the most important determinants of local preparedness activity, but Centers for Disease Control and Prevention preparedness funding plays a mediating role by building capacity through the hiring of one key leadership position, the emergency preparedness coordinator. Additional research is needed to determine the potential impact of these funds on other aspects of the local public health system, such as the scope of services delivered, to determine secondary effects of the program.

DOE Partnerships with States, Tribes and Other Federal Programs Help Responders Prepare for Challenges Involving Transport of Radioactive Materials Implementing adequate institutional programs and validating preparedness for emergency response to radiological transportation incidents along or near U.S. Department of Energy (DOE) shipping corridors poses unique challenges to transportation operations management. Delayed or insufficient attention to State and Tribal preparedness needs may significantly impact the transportation operations schedule and budget. The DOE Transportation Emergency Preparedness Program (TEPP) has successfully used a cooperative planning process to develop strong partnerships with States, Tribes, Federal agencies and other national programs to support responder preparednessmore » across the United States. DOE TEPP has found that building solid partnerships with key emergency response agencies ensures responders have access to the planning, training, technical expertise and assistance necessary to safely, efficiently and effectively respond to a radiological transportation accident. In 1990 the New Technology Demonstration Program (formerly the Test Bed Demonstration Program) was initiated by the US Department of Energy`s Office (DOE`s) of Federal Energy Management Programs with the purpose of accelerating the introduction of new technologies into the Federal sector. The program has since expanded into a multi-laboratory collaborative effort that evaluates new technologies and shares the results with the Federal design and procurement communities. Older Workers and Federal Work Programs: The Korean Senior Employment Program (KSEP).

Federal older worker programs are attracting attention due to the growing number of older workers across the world. They are uniquely situated to provide out-of-market work opportunities to older job seekers, who often find their age a barrier to securing desirable jobs. In 2004, the Korean government established its own program, the Korean Senior Employment Program (KSEP); however, literature for international readers on this innovative program is lacking. Thus, this article aims to provide an in-depth description of KSEP and a brief comparison between the Senior Community Service Employment Program in the U.S. and KSEP. The unique characteristics of KSEP include having the dual program foci on supplemental income and social participation; expanding work opportunities in the private sector beyond community-based jobs; accepting participants who are financially disadvantaged as well as those with a high desire for social participation regardless of their income; and broadening work opportunities for those with professional skills beyond repetitive, simple, and temporary jobs. This article may offer helpful insights to older worker advocates from various countries in creating or modifying their programs.

Through a cooperative effort between the U. S. Department of Energy and the Russian Federation (RF) Ministry of Defense (MOD) a Personnel Reliability Program (PRP) for the nuclear handlers within the RF MOD has been implemented. A key element in the RF MOD PRP is the detection and confirmation of narcotic use in subject military and civilian personnel. This paper describes the process of narcotics screening and testing in the RF MOD and explains the confirmation process once screening has shown a positive result. Issues of laboratory certification, employee certification, employee training, sample chain-of-custody, and equipment needs will be addressed.

The U.S. Geological Survey Federal-State Cooperative Water Resources Program is a partnership between the Geological Survey and State and local agencies for the collection of the hydrologic information needed for the continuing determination and evaluation of the quantity, quality, and use of the Nation 's water resources. The Cooperative Program has served the Nation for more than 80 years, and in 1981 more than 800 State and local agencies have cooperative programs with the Geological Survey with total funding over $80 million. The process of project selection in the Cooperative Water Resources Program is a mutual effort in which Geological Survey represents national interests, including the needs of other Federal agencies, and the cooperator represents State and local interests. The result is a balanced program that involves careful evaluation of needs, priorities, and resources. The cost sharing ratio of 50-50 is examined and determined to be the best ratio to effectively assess the Nation 's water resources. The Cooperative Program is and has been relevant to the problems of the day. The Defense Minerals Administration (DMA), Defense Minerals Exploration Administration (DMEA), and Office of Minerals Exploration (OME) mineral exploration programs were active over the period 1950–1974. Under these programs, the Federal Government contributed financial assistance in the exploration for certain strategic and critical minerals. The information about a mining property that was collected under these programs was placed in files called dockets. A docket is a collection of material (application, contract, correspondence, maps, reports, results) about a property for which an individual applied for exploration assistance from the Federal Government. Information found in dockets describe where mineral deposits were examined, what was found, and whether it was mined. As such, they provide very useful information to private industry regarding potential and non-potential prospect areas, provide the U.S. Geological Survey with useful information on mineral occurrences that are used in national assessments for particular mineral deposits, and provide other U.S. Federal agencies (Bureau of Indian Affairs, Bureau of Land Management, and Environmental Protection Agency) information relevant to land management, permitting, and leasing.

Keys to the House: Unlocking Residential Savings With Program Models for Home Energy Upgrades

After more than 40 years of effort, energy efficiency program administrators and associated contractors still find it challenging to penetrate the home retrofit market, especially at levels commensurate with state and federal goals for energy savings and emissions reductions. Residential retrofit programs further have not coalesced around a reliably successful model. They still vary in design, implementation and performance, and they remain among the more difficult and costly options for acquiring savings in the residential sector. If programs are to contribute fully to meeting resource and policy objectives, administrators need to understand what program elements are key to acquiring residentialmore » savings as cost effectively as possible. To that end, the U.S. Department of Energy (DOE) sponsored a comprehensive review and analysis of home energy upgrade programs with proven track records, focusing on those with robustly verified savings and constituting good examples for replication. The study team reviewed evaluations for the period 2010 to 2014 for 134 programs that are funded by customers of investor-owned utilities. All are programs that promote multi-measure retrofits or major system upgrades. We paid particular attention to useful design and implementation features, costs, and savings for nearly 30 programs with rigorous evaluations of performance. This meta-analysis describes program models and implementation strategies for (1) direct install retrofits; (2) heating, ventilating and air-conditioning (HVAC) replacement and early retirement; and (3) comprehensive, whole-home retrofits. We analyze costs and impacts of these program models, in terms of both energy savings and emissions avoided. These program models can be useful guides as states consider expanding their strategies for acquiring energy savings as a resource and for emissions reductions. The evaluation of educational programs has become an expected part of medical education. At some point, all medical educators will need to critically evaluate the programs that they deliver. However, the evaluation of educational programs requires a very different skillset than teaching. In this article, we aim to identify and summarize key papers that would be helpful for faculty members interested in exploring program evaluation. In November of 2016, the 2015-2016 Academic life in emergency medicine (ALiEM) Faculty Incubator program highlighted key papers in a discussion of program evaluation. This list of papers was augmented with suggestions by guest experts and by an open call on Twitter. This resulted in a list of 30 papers on program evaluation. Our authorship group then engaged in a process akin to a Delphi study to build consensus on the most important papers about program evaluation for medical education faculty. We present our group's top five most highly rated papers on program evaluation. We also summarize these papers with respect to their relevance to junior medical education faculty members and faculty developers. Program evaluation is challenging. The described papers will be informative for junior faculty members as they aim to design literature-informed evaluations for their educational programs.

Federal Occupational Health (FOH) administers a nationwide public access defibrillation program in US federal buildings. We describe the use of automated external defibrillators (AEDs) in federal buildings and evaluate survival after cardiac arrest. Using the FOH database, we examined reported events in which an AED was brought to a medical emergency in federal buildings over a 14-year period, from 1999 to 2012. There were 132 events involving an AED, 96 (73%) of which were due to cardiac arrest of cardiac etiology. Of 54 people who were witnessed to experience a cardiac arrest and presented with ventricular fibrillation or ventricular tachycardia, 21 (39%) survived to hospital discharge. Public access defibrillation, along with protocols to install, maintain, and deploy AEDs and train first responders, benefits survival after cardiac arrest in the workplace.

This article investigates the critical issues of WWPs in the Canadian context from the perspective of key stakeholders. Proposed U.S. Space Weather Budget for Fiscal Year 2011 Would Fund Key Programs

The proposed U.S. federal budget for space weather research for fiscal year (FY) 2011 would provide funding for key space weather programs within several U.S. agencies, including NASA, NOAA, the National Science Foundation (NSF), and the Air Force. Funding for the programs comes ahead of the upcoming solar maximum, a period of the solar cycle with heightened solar activity, projected for 2013. Several officials indicated that while funding is not tied to a particular solar maximum or minimum, available assets could help with studying and preparing for the solar maximum. The proposed FY 2011 budget for the Heliophysics Division within NASA's Science Mission Directorate is $641.9 million, compared with the FY 2010 enacted budget of $627.4 million. Within the proposed budget is $166.9 million for heliophysics research, down slightly from $173 million for FY 2010. The proposed budget would include $31.7 million for heliophysics research and analysis (compared with $31 million for FY 2010); $66.7 million for “other missions and data analysis,” including Cluster II, the Advanced Composition Explorer (ACE), and the Time History of Events and Macroscale Interactions during Substorms (THEMIS) mission; and $48.9 million for sounding rockets.

The federal coal leasing program implemented in the 1970s has not achieved its stated objectives and has resulted in a misallocation of resources. As of 1984, federal coal still had not played a major role in meeting either total national energy supply demands or even in meeting total coal supply demands. Federal coal accounted for only 12% of the total US production in 1982. The author notes the program has failed to coordinate coal development with other government objectives such as environmental protection; the program, moreover, has not achieved a satisfactory return to the federal treasury. He feels all threemore » branches of government must share the blame for failure of the program. The Department of the Interior's (USDI) attempts to develop a national coal leasing program failed in part because USDI could not agree on a set of workable and consistent conceptual underpinnings for the program. Administrative attempts also failed because of well-meaning but inept judicial intervention and political and economic events beyond the Department's control. Congress must also bear a good part of the blame. Congress never settled on a consistent coal use policy, and finally, after 1982-83, Congress lost faith in the USDI and attempted to legislate its own federal policy. The failure of the federal government to induce the greater use of federal coal is especially striking because ownership would seem to predict greater control over the use of the resources. 76 references.« less

To discuss changes in the law that allow community pharmacy loyalty programs to include and offer incentives to Medicare and Medicaid beneficiaries. The retailer rewards exception of the Patient Protection and Affordable Care Act of 2010 and its change to the definition of remuneration in the civil monetary penalties of the Anti-Kickback Statute now allow incentives to be earned on federal benefit tied prescription out-of-pocket costs. The criteria required to design a compliant loyalty program are discussed. Community pharmacies can now include Medicare and Medicaid beneficiaries in compliant customer loyalty programs, where allowed by state law. There is a need for research directly on the influence of loyalty programs and nominal incentives on adherence.

We aimed to determine whether syringe exchange programs (SEPs) currently receive or anticipate pursuing federal funding and barriers to funding applications following the recent removal of the long-standing ban on using federal funds for SEPs. We conducted a telephone-administered cross-sectional survey of US SEPs. Descriptive statistics summarized responses; bivariate analyses examined differences in pursuing funding and experiencing barriers by program characteristics. Of the 187 SEPs (92.1%) that responded, 90.9% were legally authorized. Three received federal funds and 116 intended to pursue federal funding. Perceived federal funding barriers were common and included availability and accessibility of funds, legal requirements such as written police support, resource capacity to apply and comply with funding regulations, local political and structural organization, and concern around altering program culture. Programs without legal authorization, health department affiliation, large distribution, or comprehensive planning reported more federal funding barriers. Policy implementation gaps appear to render federal support primarily symbolic. In practice, funding opportunities may not be available to all SEPs. Increased technical assistance and legal reform could improve access to federal funds, especially for SEPs with smaller capacity and tenuous local support.

The Herschel Space Observatory, one of the cornerstone missions of the European Space Agency (ESA) with participation from NASA, is delivering a wealth of far-infrared and sub-millimeter observations of the cold Universe. A considerable part of the observing time for the nominal three year mission lifetime has been awarded in the form of Key Programs. Between the 42 key programs (guaranteed and open times), only two key programs are dedicated to study the Solar System: "Water and Related Chemistry in the Solar System", also known as Herschel Solar System Observations (HssO) project [1], and "TNOs are Cool: A Survey of the Transneptunian Region" [2]. In the framework of these Programs, a serie of public outreach activities and efforts of its results are being carried out. We present some of the outreach strategies developed (e.g. press releases, web pages, logos, public lectures, exhibitions, interviews, reports, etc.) and some plans in this direction. Our activities introduce people to knowledge and beauty of solar system research and wider the opportunities for the public to become more involved in topics like solar system studies, specially in the times of frequent exo-planet discoveries.

This paper documents nearly a half century of U.S. federal government support for energy research and development (R&D). Data on energy R&D expenditures disaggregated by major program area are presented here for the first time for the period 1961-2008. This paper also documents U.S. federal government spending on key large scale energy R&D programs that were initiated in response to the oil crisis of the 1970s. Since 1961, the U.S. government has invested nearly $172 billion (in inflation adjusted 2005 US dollars) for the development of advanced energy technologies and for the necessary underlying basic science. The US Government (USG) can date its involvement with immunization to military and civilian efforts in 1777 and 1813 to prevent smallpox. USG involvement began accelerating with federal licensing of vaccine and antibody manufacturers in 1903. In addition to ongoing regulation of manufacturing and product quality, military and civilian arms of the USG have led research efforts into new or improved vaccines. These efforts have included diseases endemic in the United States, as well as medical countermeasures targeted against biological weapons, influenza pandemics, and emerging infectious diseases. Especially since the 1950s, the USG has provided increasing levels of funding to purchase vaccines and conduct vaccination programs. These programs have focused largely on children, although vaccination programs for adults have been expanded somewhat in recent years. Multiple agencies of the USG have convened various panels of accomplished external experts who have generated widely regarded recommendations on vaccine safety and efficacy and optimal immunization practices. USG programs for safety assessment, injury compensation, liability protection, and disease surveillance have been developed to assess needs, evaluate safety questions, ensure vaccine supply, and foster confidence in vaccination efforts. Debates on the extent of government involvement date back to the 1890 s and continue today. Several pivotal expansions of government involvement followed disease outbreaks or manufacturing accidents. This action amends the rule to create a regulatory exception that allows children of same-sex domestic partners living overseas to maintain their Federal Employees Health Benefits (FEHB) and Federal Employees Dental and Vision Program (FEDVIP) coverage until September 30, 2018. Due to a recent Supreme Court decision, as of January 1, 2016, coverage of children of same-sex domestic partners under the FEHB Program and FEDVIP will generally only be allowed if the couple is married, as discussed in Benefits Administration Letter (BAL) 15-207 dated October 5, 2015. OPM recognizes there are additional requirements placed on overseas federal employees that may not apply to other civilian employees with duty stations in the United States making it difficult to travel to the United States to marry same-sex partners.

No-signaling quantum key distribution: solution by linear programming

We outline a straightforward approach for obtaining a secret key rate using only no-signaling constraints and linear programming. Assuming an individual attack, we consider all possible joint probabilities. Initially, we study only the case where Eve has binary outcomes, and we impose constraints due to the no-signaling principle and given measurement outcomes. Within the remaining space of joint probabilities, by using linear programming, we get bound on the probability of Eve correctly guessing Bob's bit. We then make use of an inequality that relates this guessing probability to the mutual information between Bob and a more general Eve, who is not binary-restricted. Putting our computed bound together with the Csiszár-Körner formula, we obtain a positive key generation rate. The optimal value of this rate agrees with known results, but was calculated in a more straightforward way, offering the potential of generalization to different scenarios.

Probabilities of gene origin computed from the genomic kinships matrix can accurately identify key ancestors of modern germplasms Identifying the key ancestors of modern plant breeding populations can provide valuable insights into the history of a breeding program and provide reference genomes for next generation whole genome sequencing. In an animal breeding context, a method was developed that employs probabilities of gene origin, computed from the pedigree-based additive kinship matrix, for identifying key ancestors. Because reliable and complete pedigree information is often not available in plant breeding, we replaced the additive kinship matrix with the genomic kinship matrix. As a proof-of-concept, we applied this approach to simulated data sets with known ancestries. The relative contribution of the ancestral lines to later generations could be determined with high accuracy, with and without selection. Our method was subsequently used for identifying the key ancestors of the modern Dent germplasm of the public maize breeding program of the University of Hohenheim. We found that the modern germplasm can be traced back to six or seven key ancestors, with one or two of them having a disproportionately large contribution. These results largely corroborated conjectures based on early records of the breeding program. We conclude that probabilities of gene origin computed from the genomic kinships matrix can be used for identifying key ancestors in breeding programs and estimating the proportion of genes contributed by them.

Developing physician-leaders: key competencies and available programs.

Because effective leadership is critical to organizational success, frontrunner organizations cultivate leaders for bench depth and pipeline development. The many challenges in healthcare today create a special need for great leadership. This paper reviews the leadership competencies needed by physician-leaders and current experience with developing physician-leaders in healthcare institution-sponsored programs. On the basis of this review, six key leadership competency domains are proposed: 1. technical skills and knowledge (regarding operational, financial, and information systems, human resources, and strategic planning), 2. industry knowledge (e.g., regarding clinical processes, regulation, and healthcare trends), 3. problem-solving skills, 4. emotional intelligence, 5. communication, and 6. a commitment to lifelong learning. Review of current experience indicates that, in addition to leadership training through degree and certificate-granting programs (e.g., by universities and/or official medical societies), healthcare institutions themselves are developing intramural programs to cultivate physician-leaders. Greater attention is needed to assessing the impact and effectiveness of such programs in developing leaders and benefiting organizational outcomes.

Federal immunization policy and funding: a history of responding to crises.

This article outlines the history of federal immunization policy and funding, with a focus on discretionary federal funding under Section 317 of the Public Health Service Act, paying particular attention to the role of Congress in shaping the program in the past 2 decades. This review of funding trends and initiatives indicates that when both a presidential administration and key congressional actors viewed immunization as a priority and made sufficient funds available to support the public health delivery system and its infrastructure, coverage levels would continue to rise and disease levels continue to decline. From the beginning, immunization financing was explicitly structured as a federal-state-private-sector partnership. Section 317 program's statute has not changed much in 35 years, despite significant changes to the health care delivery system, other federal immunization activities, and rates of immunization coverage. Although the creation and implementation of the Vaccines for Children (VFC) program in the mid-1990s resulted in some congressional deliberations over immunization policies, no explicit restructuring of the 317 program occurred as a result. The Section 317 program retains its traditional authority and mission to address urgent needs, sustain public delivery systems, and provide funds for purchase of vaccines. The question remains whether the resources to sustain progress in immunization can be secured during times with no crisis, to ensure constant "readiness" in immunization (as in defense), or whether another epidemic must occur before the federal government is willing to commit optimal resources.

To be successful, Early Hearing Detection and Intervention (EHDI) programs require individually identifiable information about children to be shared among people who are responsible for screening, diagnosis, early intervention, family support, and medical home services. Pediatricians and other stakeholders in the EHDI process often point to federal laws that were passed to ensure privacy and confidentiality in health care and educational programs as major obstacles to achieving efficient and effective EHDI programs. In this article we summarize the provisions of 3 federal laws (the Health Insurance Portability and Accountability Act [HIPAA], the Family Education Rights and Privacy Act [FERPA], and Part C privacy regulations of the Individuals With Disabilities Education Act [IDEA]) that most directly affect information-sharing in EHDI programs. We suggest strategies for sharing the information needed to operate successful EHDI programs while remaining in compliance with these laws, including obtaining signed parental consent to share information between providers, including an option on the individual family services plan for parents to permit sharing of the plan with pediatricians and other providers, and giving copies of all relevant test results to parents to share with providers as they wish.

Key ingredients of anti-stigma programs for health care providers: a data synthesis of evaluative studies.

As part of its ongoing effort to combat stigma against mental illness among health care providers, the Mental Health Commission of Canada partnered with organizations conducting anti-stigma interventions. Our objective was to evaluate program effectiveness and to better understand what makes some programs more effective than others. Our paper reports the elements of these programs found to be most strongly associated with favourable outcomes. Our study employed a multi-phased, mixed-methods design. First, a grounded theory qualitative study was undertaken to identify key program elements. Next, each program (n = 22) was coded according to the presence or absence of the identified key program ingredients. Then, random-effects, meta-regression modelling was used to examine the association between program outcomes and the key ingredients. The qualitative analysis led to a 6-ingredient model of key program elements. Results of the quantitative analysis showed that programs that included all 6 of these ingredients performed significantly better than those that did not. Individual analyses of each of the 6 ingredients showed that including multiple forms of social contact and emphasizing recovery were characteristics of the most effective programs. The results provide a validation of a 6-ingredient model of key program elements for anti-stigma programming for health care providers. Emphasizing recovery and including multiple types of social contact are of particular importance for maximizing the effectiveness of anti-stigma programs for health care providers.

A concept for performance management for Federal science programs

The demonstration of clear linkages between planning, funding, outcomes, and performance management has created unique challenges for U.S. Federal science programs. An approach is presented here that characterizes science program strategic objectives by one of five “activity types”: (1) knowledge discovery, (2) knowledge development and delivery, (3) science support, (4) inventory and monitoring, and (5) knowledge synthesis and assessment. The activity types relate to performance measurement tools for tracking outcomes of research funded under the objective. The result is a multi-time scale, integrated performance measure that tracks individual performance metrics synthetically while also measuring progress toward long-term outcomes. Tracking performance on individual metrics provides explicit linkages to root causes of potentially suboptimal performance and captures both internal and external program drivers, such as customer relations and science support for managers. Functionally connecting strategic planning objectives with performance measurement tools is a practical approach for publicly funded science agencies that links planning, outcomes, and performance management—an enterprise that has created unique challenges for public-sector research and development programs.

The U.S. Geological Survey 's Federal-State Cooperative Water Resources Program (50-50 matching of funds) started in Kansas in 1895. During fiscal year (FY) 1987, hydrologic data collection, investigations, and research are being conducted in every state, Puerto Rico, and several territories in cooperation with 940 state, regional and local agencies. Federal funding of $55.3 million was matched by cooperating agencies; cooperators also provided $4.6 million unmatched, for a program total of about $115 million. The Cooperative Program accounted for almost 45% of the FY 1987 obligations of the Geological Survey 's Water Resources Division. The principal areas of emphasis during the year included groundwater contamination, stream quality, water supply and demand, and hydrologic hazards. Information is presented on program functions and priorities. Data collection activities are also described as is work related to water resources contamination. Since the 1970s, the federal government has spearheaded major national education programs to reduce the burden of chronic diseases in the United States. These prevention and disease management programs communicate critical information to the public, those affected by the disease, and health care providers. The National Diabetes Education Program (NDEP), the leading federal program on diabetes sponsored by the National Institutes of Health (NIH) and the Centers for Disease Control and Prevention (CDC), uses primary and secondary quantitative data and qualitative audience research to guide program planning and evaluation. Since 2006, the NDEP has filled the gaps in existing quantitative data sources by conducting its own population-based survey, the NDEP National Diabetes Survey (NNDS). The NNDS is conducted every 2–3 years and tracks changes in knowledge, attitudes and practice indicators in key target audiences. This article describes how the NDEP has used the NNDS as a key component of its evaluation framework and how it applies the survey results for strategic planning and program improvement. The NDEP's use of the NNDS illustrates how a program evaluation framework that includes periodic population-based surveys can serve as an evaluation model for similar national health education programs.

Could a federal program to promote influenza vaccination among elders be cost-effective?

Influenza-related mortality predominately and disproportionately impacts the elderly. Rates of annual influenza vaccination among the elderly are approximately 65%, far below the Healthy People 2010 target of 90%. We estimated the cost-effectiveness of a 10-year federal program to promote influenza vaccine, intended to increase vaccination rates among persons > or = 65 years old. Published estimates regarding influenza-associated mortality rates and vaccine efficacy among the US elderly were used to calculate the number needed to vaccinate (NNV) to prevent one all-cause death due to influenza, as well as the mortality reduction expected from increased vaccination rates. The costs per life-year saved were estimated for a hypothetical federal promotional campaign, patterned after a direct-to-consumer (DTC) advertising program (2006-2015). The base case scenario presumed a 25-percentage-point increase in vaccination rates to 90%; in sensitivity analyses, we examined programs that increased rates by 10-20 points. The base case NNV was 1116 (95% CI: 993-1348). Over the 10-year DTC-style influenza vaccine promotion program, 6516 (5576-7435) elderly lives would be saved. The incremental cost-effectiveness (C/E) of the program was dollar 16,300 (dollar 11,347-dollar 25,174) per life-year saved in 2006 and increased to dollar 199,906 (dollar 138,613-dollar 307,423) per life-year saved by 2015. Overall, the C/E for the 10-year program was dollar 37,621 (dollar 32,644-dollar 43,939) per life-year saved. Programs that yielded a 15-percentage-point increase or less in vaccination rates would have C/E values exceeding dollar 50,000 per life-year saved and save fewer than 4000 total lives. DTC-style promotional campaigns for influenza vaccine among elders may represent a cost-effective strategy for the federal government to pursue as a means of increasing elders' vaccination rates and reducing influenza-related mortality.

Highlights of the 1983 Federal-state cooperative water resources program

The U.S. Geological Survey Federal-State Cooperative Water Resources Program in fiscal year 1983 continued to concentrate on investigations of highest priority to the Nation. Hydrologic data collection and interpretive studies were underway in every State, Puerto Rico, and several U.S. territories with focus on such current concerns as ground-water contamination, floods, impacts of toxic wastes, acid precipitation, and stream quality. During the year, this 50-50 matching program was carried out in working partnership with more than 800 State, regional, and local agencies. Joint funding from all sources totaled approximately $92 million. Details of the program are mutually negotiated at the working level by representatives of the Survey and representatives of the cooperating agencies. The pooling of interests results in a balanced effort that directs combined resources to hydrologic investigations having the most significance to both parties. A few of the highlights for FY 1983, and how the program is developed with other agencies are described. (USGS)

States' experiences with loan repayment programs for health care professionals in a time of state budget cuts and NHSC expansion.

The landscape of education loan repayment programs for health care professionals has been turbulent in recent years, with doubling of the funding for the National Health Service Corps (NHSC) and cuts in funding for some states' programs. We sought to understand how this turbulence is being felt within the state offices involved in recruiting clinicians to rural and urban underserved communities. We conducted key informant telephone interviews with staff of state offices of rural health, primary care organizations, and/or related organizations within 28 diverse states to answer questions about perceived changes and interplay among solely state-funded loan repayment programs, joint state-federal programs, and the NHSC federal program. Interviews were transcribed, formally analyzed, and key issues summarized. Informants reported that solely state-funded and joint state-federal loan repayment programs are greatly valued for their ability to target a state's particular needs and to complement the NHSC federal program. However, budgets for state programs have been threatened, reduced, or eliminated entirely in many cases. All informants positively perceived the NHSC's recent growth and changes, which they feel are helping fill important workforce needs for their states. Nevertheless, the much larger NHSC federal program now competes with some states' programs for clinicians and service sites; states' programs are pushed to adjust their operations to maintain a unique "niche". States' key recruiters lament reductions in funding for states' loan repayment programs, and welcome the NHSC's recent growth and changes. Better coordination is needed to minimize competition and maximize complementarity between state and federal programs. © 2012 National Rural Health Association.

Beaumont is a multiple hospital health care system with a centralized radiation safety department. The health system operates under a broad scope Nuclear Regulatory Commission license but also maintains several other limited use NRC licenses in off-site facilities and clinics. The hospital-based program is expansive including diagnostic radiology and nuclear medicine (molecular imaging), interventional radiology, a comprehensive cardiovascular program, multiple forms of radiation therapy (low dose rate brachytherapy, high dose rate brachytherapy, external beam radiotherapy, and gamma knife), and the Research Institute (including basic bench top, human and animal). Each year, in the annual report, data is analyzed and then tracked and trended. While any summary report will, by nature, include items such as the number of pieces of equipment, inspections performed, staff monitored and educated and other similar parameters, not all include an objective review of the quality and effectiveness of the program. Through objective numerical data Beaumont adopted seven key performance indicators. The assertion made is that key performance indicators can be used to establish benchmarks for evaluation and comparison of the effectiveness and quality of radiation safety programs. Based on over a decade of data collection, and adoption of key performance indicators, this paper demonstrates one way to establish objective benchmarking for radiation safety programs in the health care environment.

Federal public health programs use a mix of formula-based and competitive methods to allocate funds among states and other constituent jurisdictions. Characteristics of formula-based allocations used by a convenience sample of four programs, three from the Centers for Disease Control and Prevention and one from the Health Resources and Services Administration, are described to illustrate formula-based allocation methods in public health. Data sources in these public health formulas include population counts and funding proportions based on historical precedent. None include factors that adjust allocations based on variations in the availability of local resources or the cost of delivering services. Formula-funded activities are supplemented by programs that target specific prevention needs or encourage development of innovative methods to address emerging problems, using set-aside funds. A public health finance research agenda should address ways to improve the fit between funding allocation formulas and program objectives.

Transforming a family medicine center and residency program into a federally qualified health center.

The authors describe a family medicine center before and after a merger between the Keck School of Medicine of the University of Southern California, the California Hospital Medical Center, and the Eisner Pediatric and Family Medical Center in 2012. The merger provided new opportunities to stabilize the financial base of a clinical practice struggling financially and to enhance the training of residents and other health professionals in primary care, which motivated the partners to consider this new model. After 18 months of negotiations, they were able to convert the family medicine center and residency program into a new federally qualified health center. The benefits to this new model include an increase in both patient volume and the quality of education, supporting residency accreditation; a greater number of residents from U.S. medical schools; enhanced education and preparation of primary care physicians for practice in medically underserved communities; enhanced reimbursements and new opportunities for state, local, and federal grants; and quality improvement and new information technology. The partners overcame academic, administrative, legal, and regulatory obstacles, communication barriers, and differences in culture and expectations to achieve this merger. Keys to their success include the commitment of the leaders at the three institutions to the goals of the merger, a dedicated project manager and consultants, opportunities for new revenue sources and reimbursements, and support from a pioneering charitable foundation. The authors conclude by discussing the implications of using community health centers as the focal point for training primary care clinicians and addressing workforce shortages.

Since 1987, Clark County has been designated by the U.S. Department of Energy (DOE) as an 'Affected Unit of Local Government' (AULG). The AULG designation is an acknowledgement by the federal government that activities associated with the Yucca Mountain proposal could result in considerable impacts on Clark County residents and the community as a whole. As an AULG, Clark County is authorized to identify 'any potential economic, social, public health and safety, and environmental impacts of a repository', 42 U.S.C. Section 10135(c)(1)(B)(i) under provisions of the Nuclear Waste Policy Act Amendments (NWPAA). Clark County's oversight program contains key elements ofmore » (1) technical and scientific analysis (2) transportation analysis (3) impact assessment and monitoring (4) policy and legislative analysis and monitoring, and (5) public outreach. Clark County has conducted numerous studies of potential impacts, many of which are summarized in Clark County's Impact Assessment Report that was submitted DOE and the President of the United States in February 2002. Given the unprecedented magnitude and duration of DOE's proposal, as well as the many unanswered questions about the transportation routes, number of shipments, and the modal mix that will ultimately be used, impacts to public health and safety and security, as well as socioeconomic impacts, can only be estimated. In order to refine these estimates, Clark County Comprehensive Planning Department's Nuclear Waste Division updates, assesses, and monitors impacts on a regular basis. Clark County's Impact Assessment program covers not only unincorporated Clark County but all five jurisdictions of Las Vegas, North Las Vegas, Henderson, Mesquite, and Boulder City as well as tribal jurisdictions that fall within Clark County's geographic boundary. This paper highlights the value of utilizing a participatory evaluation approach when working with community agencies receiving federal funding for prevention and intervention services. Drawing from our experience as evaluators of a SAMHSA-funded substance abuse, HIV and Hepatitis prevention program targeting homeless young adults, we describe the importance of and strategies for creating a participatory evaluation partnership with program implementers. By participatory evaluation we mean the active involvement of program implementers in defining the evaluation, developing instruments, collecting data, discussing findings, and disseminating results. There are a number of challenges faced when using this approach with federally funded programs that require the use of standardized measurement tools and data collection procedures. Strategies we used to strike a balance between federal requirements and local needs are presented. By increasing the understanding of and participation in the evaluation process, program implementers have greater support for data collection requirements and are appreciably more interested in learning from the evaluation data. This approach has helped to build the capacity of a program and stimulated new possibilities for learning, growing, and ultimately improving the services offered to those the program strives to reach. Increasing health insurance coverage through an extended Federal Employees Health Benefits Program.

The Federal Employees Health Benefits Program (FEHBP) could be combined with health insurance tax credits to extend coverage to the uninsured. An extended FEHBP, or "E-FEHBP," would be open to all individuals who were not covered through work or public programs and who also were eligible for the tax credits on the basis of income. E-FEHBP also would be open to employees of very small firms, regardless of their eligibility for tax credits. Most plans available to FEHBP participants would be required to offer enrollment to E-FEHBP participants, although premiums would be rated separately. High-risk individuals would be diverted to a separate high-risk pool, the cost of which would be subsidized by the federal government. E-FEHBP would be administered by the states, or if a state declined, by an entity that contracted with the Office of Personnel Management. While E-FEHBP would provide group insurance to people who otherwise could not get it, premiums could exceed the tax-credit amount and some people still might find the coverage unaffordable.

Research supports the effectiveness of the Risk-Needs-Responsivity model for reducing criminal recidivism. Yet programming interests of inmates--one facet of responsivity--remain an understudied phenomenon. In the present study, we explored the programming interests of 753 federal inmates housed across three levels of security. Results suggest that inmates, as a group, prefer specific programs over others, and that some of their interests may differ by security level. To accomplish Federal goals for renewable energy, sustainability, and energy security, large-scale renewable energy projects must be developed and constructed on Federal sites at a significant scale with significant private investment. For the purposes of this Guide, large-scale Federal renewable energy projects are defined as renewable energy facilities larger than 10 megawatts (MW) that are sited on Federal property and lands and typically financed and owned by third parties.1 The U.S. Department of Energy’s Federal Energy Management Program (FEMP) helps Federal agencies meet these goals and assists agency personnel navigate the complexities of developing such projects and attract the necessarymore » private capital to complete them. This Guide is intended to provide a general resource that will begin to develop the Federal employee’s awareness and understanding of the project developer’s operating environment and the private sector’s awareness and understanding of the Federal environment. Because the vast majority of the investment that is required to meet the goals for large-scale renewable energy projects will come from the private sector, this Guide has been organized to match Federal processes with typical phases of commercial project development. FEMP collaborated with the National Renewable Energy Laboratory (NREL) and professional project developers on this Guide to ensure that Federal projects have key elements recognizable to private sector developers and investors. The Teaching Health Center Graduate Medical Education (THCGME) program is a decentralized residency training component of the Affordable Care Act, created to combat critical shortages and maldistribution of primary care physicians. The Accreditation Council of Graduate Medical Education and federal data reveal that the THCGME program accounted for 33% of the net increase in family medicine residency positions between 2011 and 2015. However, amid concerns about the program's stability, the contribution of the THCGME program to the net increase fell to 7% after 2015. © Copyright 2017 by the American Board of Family Medicine.

Challenges and strategies in applying performance measurement to federal public health programs.

Performance measurement is widely accepted in public health as an important management tool supporting program improvement and accountability. However, several challenges impede developing and implementing performance measurement systems at the federal level, including the complexity of public health problems that reflect multiple determinants and involve outcomes that may take years to achieve, the decentralized and networked nature of public health program implementation, and the lack of reliable and consistent data sources and other issues related to measurement. All three of these challenges hinder the ability to attribute program results to specific public health program efforts. The purpose of this paper is to explore these issues in detail and offer potential solutions that support the development of robust and practical performance measures to meet the needs for program improvement and accountability. Adapting performance measurement to public health programs is both an evolving science and art. Through the strategies presented here, appropriate systems can be developed and monitored to support the production of meaningful data that will inform effective decision making at multiple levels.
