These final regulations implement a new Income-Contingent Repayment (ICR) plan in the Direct Loan program based on the President's ``Pay As You Earn'' repayment initiative, incorporate recent statutory changes to the Income-Based Repayment (IBR) plan in the Direct Loan and FFEL programs, and streamline and add clarity to the total and permanent disability (TPD) discharge process for borrowers in loan programs under title IV of the Higher Education Act of 1965, as amended (HEA). These final regulations implementing a new ICR plan and the statutory changes to the IBR plan will assist borrowers in repaying their loans while the changes to the TPD discharge process will reduce burden for borrowers who are disabled and seeking a discharge of their title IV debt.

The effects of corporate restructuring on hospital policymaking.

Hospital corporate restructuring is the segmentation of assets or functions of the hospital into separate corporations. While these functions are almost always legally separated from the hospital, their impact on hospital policymaking may be far more direct. This study examines the effects of corporate restructuring by community hospitals on the structure, composition, and activity of hospital governing boards. In general, we expect that the policymaking function of the hospital will change to adapt to the multicorporate structure implemented under corporate restructuring, as well as the overlapping boards and diversified business responsibilities of the new corporate entity. Specifically, we hypothesize that the hospital board under corporate restructuring will conform more to the "corporate" model found in the business/industrial sector and less to the "philanthropic" model common to most community hospitals to date. Analysis of survey data from 1,037 hospitals undergoing corporate restructuring from 1979-1985 and a comparison group of 1,883 noncorporately restructured hospitals suggests general support for this hypothesis. Implications for health care governance and research are discussed. PMID:3384671

As a result of the EU Tissues and Cells Directive (2004/23/EC), therapeutic tissue banking is currently being restructured throughout Europe. The stated objectives are to enhance a safe and stable supply of bone and tissue in Europe and to facilitate internal exchange. We conducted an interview study to explore the effect of the Directive on Danish bone banks in terms of (1) organizational restructuring, (2) supply and range of exchange, (3) economic costs. We found that the Directive stimulated extensive re-organization of bone banks with a substantial adjoining workload; that it is doubtful whether it will increase supply and range of exchange; and that the transposition of the Directive is associated with considerable extra cost. Additionally, we found that elements in the documentation of safety were fabricated by surgeons to avoid what was seen as unnecessary questioning of potential donors.

In Sickness and in Debt: Do Mounting Medical Bills Predict Payday Loan Debt?

Cash-strapped families sometimes turn to small, short-term loans with exorbitant fees—payday loans—to cope with mounting medical bills. Given that about three-fourths of payday loan customers are repeat borrowers, consumer advocates and policymakers have increasingly raised voices of concern about the use of payday loans to finance various household expenses, including, among other things, medical bills. The present study hypothesized that increases in medical debt are associated with increases in payday loan debt among a sample of Chapter 7 bankruptcy filers. The results of a multivariate tobit regression analysis showed that medical debt was associated with increased payday loan debt, controlling for various types of debt and other socioeconomic variables. This article concludes with implications of the results for social work policy- and direct-practice.

Testing the tools of development: credit programmes, loan involvement, and women's empowerment.

This study finds that income generation programs should not focus on women's labor as a means of empowerment. Empowerment should be directed to women's direct involvement in selling and accounting in the loan activity and in the techniques of lending agencies that promote empowerment. Borrowers are empowered through market activities and nonmarket strategies such as changing the institutional environment. Local women can be hired as group promoters. When payments are made to a collection post, local women promoters can be protected and travel to remote areas without fear for security. It is argued that goals are reached successfully, when tasks are clearly defined. Empowerment in this study means knowledge of accounting for the borrower's loan activity. It is argued that organizations that choose to make women's empowerment a goal can use this factor to measure program impact. Data for this study are obtained from the Bangladesh Rural Advancement Committee (BRAC), Grameen Bank (GB), and Save the Children, Bangladesh Field Office (SCF). The general model indicates that the borrower will be empowered in the process of taking out a loan and investing it in an activity. The likelihood that the credit process will lead to empowerment, given the multiple lending options, is tested in a probit model. Explanatory variables include location, borrower's direct contribution of labor to all or part of the activity, a borrower's direct involvement in buying supplies or selling or accounting for the loan, the lending organization (BRAC, GB, or SCF), loan amount, number of years of borrowing, and borrower position as a center or group chair. The results are based on the 826 loans of the 613 female borrowers. Significant features that contributed to borrower's knowledge included, in the order of significance, participation in accounting, selling, and labor. The key factor appeared to be market access. The impact of organization and involvement was maximized among individuals who had a 40-50% probability of being empowered. For example, a borrower, who had an initial propensity of being empowered of 20%, would have a 96.2% probability of being empowered as a result of accounting involvement.

Here we use ozone-activated silver–gold alloys in the form of nanoporous gold as a case study to demonstrate the dynamic behaviour of bimetallic systems during activation to produce a functioning catalyst. We show that it is these dynamic changes that give rise to the observed catalytic activity. Advanced in situ electron microscopy and X-ray photoelectron spectroscopy are used to demonstrate that major restructuring and compositional changesmore » occur along the path to catalytic function for selective alcohol oxidation. Transient kinetic measurements correlate the restructuring to three types of oxygen on the surface. The direct influence of changes in surface silver concentration and restructuring at the nanoscale on oxidation activity is demonstrated. The Communications Act of 1934, which regulates the United States telecommunications industry, is becoming less able to deal with the changes brought by modern technology. Therefore, proposals are being made to restructure the industry. Breaking up AT&T into smaller firms would have only minor effects, since its ratio of R and D to sales has been near the median for large telephone companies. Restructuring AT&T into subsidiaries dealing at arms' length might cause a reordering of priorities toward more directly marketable and useful products, but too extensive a break-up could endanger Bell Labs' commitment to basic research. Regulation leads a firm to ignore true social value of possible products, expand into low profit markets, and favor capital-intensive over labor-intensive innovation. If regulatory lag occurs, a firm will produce cost-reducing innovation in order to increase its profit.

Does educational indebtedness affect physician specialty choice?

There has been much debate over the effect of educational indebtedness on the specialty choices of new physicians, especially in light of the perceived shortage of primary care physicians. This paper explores the theoretical foundations on which this debate is based. In addition, the paper estimates the effects of various types of debt on specialty choice. The results suggest that an increase in debt from subsidized loan sources (i.e., Guaranteed Student Loans, National Direct Student Loans, or Health Professions Student Loans) has mixed effects while an increase in debt from Health Education Assistance Loans reduces the likelihood of becoming a primary care physician. Though these effects are significant, they are very small in magnitude. Economic returns to certain specialties and personal background appear to play a more important role in specialty choice.

Spot market volume so far this year, by NUKEM's estimates, comes to just under 11 million lbs. It appears that existing loans are either being extended, or paid back with material borrowed from other sources. Therefore, there has been no significant amount of purchases on the spot market to pay back borrowed uranium. How do we know the loans have not been paid back with spot purchases For one thing, the amount of uranium loans outstanding has increased. According to our current survey, there may now be as much as 32 million lbs U3O8 equivalent in outstanding loans. At currentmore » prices, it's cheaper to borrow than to buy uranium. So borrowers are gambling that prices will remain low for some time, allowing them to delay repayment of the loans. Borrowers then, in essence, are delaying the day of reckoning on these loans. How long they can do so is anyone's guess. As long as uranium is in abundance and utilities remain willing to lend it out, loan activity will likely remain at or near current levels. But when supplies tighten and the market swings in a more positive direction, borrowers may get caught scrambling to repay their loans as quickly as possible.« less
