In April 1972, "Postal History International" published an article of mine concerning anti-vaccination caricature envelopes, which has led to some correspondence with readers and the discovery of several related items. It seems appropriate, therefore, to summarise the information in these pages, in the hope that this may bring forth knowledgeable comment from others. Details of any foreign propaganda of this sort would be particularly welcome. Meanwhile, this articles summarises what is known of the Society responsible for at least some of the British envelopes, and fills in a little of the historical background. Pedigrees, propaganda, and paranoia: family studies in a historical context.

This article reviews the uses of family studies carried out in the early 20th century under the banner of eugenics, a companion discipline to early genetics. It explores how, in an attempt to analyze and quantify purportedly biologic bases of social problems, the eugenicists constructed pedigree charts of notoriously "defective" families. Investigation of individuals with suspect traits formed the basis for instruction of field workers who linked those traits to larger groups. The resulting eugenic family studies provided a "scientific" face for a popular hereditarian mythology that claimed to explain all social failure in systematic terms. The eugenicists were successful in fueling public fear about the growing "army of idiots and imbeciles" graphically depicted in their pedigree charts. Their success was the result of a finely crafted educational program--propaganda that reduced science to simplistic terms. The tendency to oversimplify concepts of genetic causation and the rush to amplify the significance of research findings through the popular media is also apparent today. What begins as publicity has the potential to be transformed into propaganda. Although many in the scientific community are understandably reluctant to revisit the abuses of the past, that community must confront the history of eugenics as a necessary antidote to the genetic hype that surrounds us.

Institutions and Cultural Diversity: Effects of Democratic and Propaganda Processes on Local Convergence and Global Diversity.

In a connected world where people influence each other, what can cause a globalized monoculture, and which measures help to preserve the coexistence of cultures? Previous research has shown that factors such as homophily, population size, geography, mass media, and type of social influence play important roles. In the present paper, we investigate for the first time the impact that institutions have on cultural diversity. In our first three studies, we extend existing agent-based models and explore the effects of institutional influence and agent loyalty. We find that higher institutional influence increases cultural diversity, while individuals' loyalty to their institutions has a small, preserving effect. In three further studies, we test how bottom-up and top-down processes of institutional influence impact our model. We find that bottom-up democratic practices, such as referenda, tend to produce convergence towards homogeneity, while top-down information dissemination practices, such as propaganda, further increase diversity. In our last model--an integration of bottom-up and top-down processes into a feedback loop of information--we find that when democratic processes are rare, the effects of propaganda are amplified, i.e., more diversity emerges; however, when democratic processes are common, they are able to neutralize or reverse this propaganda effect. Importantly, our models allow for control over the full spectrum of diversity, so that a manipulation of our parameters can result in preferred levels of diversity, which will be useful for the study of other factors in the future. We discuss possible mechanisms behind our results, applications, and implications for political and social sciences.

Visualizing the Geography of the Diseases of China: Western Disease Maps from Analytical Tools to Tools of Empire, Sovereignty, and Public Health Propaganda, 1878-1929.

Argument This article analyzes for the first time the earliest western maps of diseases in China spanning fifty years from the late 1870s to the end of the 1920s. The 24 featured disease maps present a visual history of the major transformations in modern medicine from medical geography to laboratory medicine wrought on Chinese soil. These medical transformations occurred within new political formations from the Qing dynasty (1644-1911) to colonialism in East Asia (Hong Kong, Taiwan, Manchuria, Korea) and hypercolonialism within China (Tianjin, Shanghai, Amoy) as well as the new Republican Chinese nation state (1912-49). As a subgenre of persuasive graphics, physicians marshaled disease maps for various rhetorical functions within these different political contexts. Disease maps in China changed from being mostly analytical tools to functioning as tools of empire, national sovereignty, and public health propaganda legitimating new medical concepts, public health interventions, and political structures governing over human and non-human populations.

Research on parochial altruism demonstrated that hostility toward out-groups (parochialism) represents the dark side of the willingness to benefit one's in-group even at own costs (altruism). Parochial aggression thereby emerged mainly under conditions of threat. Extremist propaganda videos, for instance by right-wing extremists, try to capitalize on parochial altruistic mechanism by telling recipients sharing their national identity that this nation is under threat wherefore they for have to join the extremist's cause to prevent the extinction of their nation. Most of the time, propaganda videos are rated as uninteresting and non-persuasive by the target audience. Yet, evolutionary media psychology posits that the interest in and effectiveness of media increases when evolutionarily relevant problems are addressed. Consequently, interest in parochial altruistic right-wing extremist messages should increase under conditions of threat. The current study tested this assumption by randomly assigning German non-Muslims (N = 109) to either an existential threat (here: mortality salience) or a control condition and asking them to evaluate extremist propaganda that addressed them as either in-group members (right-wing extremists) or as out-group members (Islamic extremists). In support of the hypotheses, subjects under conditions of threat reported a higher interest in the right-wing extremist propaganda and perceived it as more persuasive. We discuss the results concerning the implications for evolutionary media psychology and the transmission of parochial altruism in propaganda videos.

The Interaction of Extremist Propaganda and Anger as Predictors of Violent Responses.

In this study, and with a view to extending upon existing findings on the effects of general violent media on violent cognitions, we experimentally measured the relationship between exposure to extremist propaganda and violent cognitions. Our results countered our hypotheses and the wider findings of violent media and aggression that exposure to violent stimuli increases violent thoughts and that this effect is moderated by trait aggression. Specifically, this study found that participants with low and medium trait aggression became more pro-social after being exposed to extremist propaganda. We discuss these results with reference to theories of terror management and mortality salience, as well as the implications of these results for wider theories of the role of online extremist material in the wider "radicalization" process.

Academic advocacy in public health: Disciplinary 'duty' or political 'propaganda'?

The role of 'advocacy' within public health attracts considerable debate but is rarely the subject of empirical research. This paper reviews the available literature and presents data from qualitative research (interviews and focus groups conducted in the UK in 2011-2013) involving 147 professionals (working in academia, the public sector, the third sector and policy settings) concerned with public health in the UK. It seeks to address the following questions: (i) What is public health advocacy and how does it relate to research?; (ii) What role (if any) do professionals concerned with public health feel researchers ought to play in advocacy?; and (iii) For those researchers who do engage in advocacy, what are the risks and challenges and to what extent can these be managed/mitigated? In answering these questions, we argue that two deeply contrasting conceptualisations of 'advocacy' exist within public health, the most dominant of which ('representational') centres on strategies for 'selling' public health goals to decision-makers and the wider public. This contrasts with an alternative (less widely employed) conceptualisation of advocacy as 'facilitational'. This approach focuses on working with communities whose voices are often unheard/ignored in policy to enable their views to contribute to debates. We argue that these divergent ways of thinking about advocacy speak to a more fundamental challenge regarding the role of the public in research, policy and practice and the activities that connect these various strands of public health research. Nationalism, racism and propaganda in early Weimar Germany: contradictions in the campaign against the "black horror on the Rhine".

During the early 1920s, an average of 25,000 colonial soldiers from North Africa, Senegal and Madagascar formed part of the French army of occupation in the Rhineland. The campaign against these troops, which used the racist epithet ‘black horror on the Rhine’ (schwarze Schmach am Rhein), was one of the most important propaganda efforts of the Weimar period. In black horror propaganda, images of alleged sexual violence against Rhenish women and children by African French soldiers served as metaphors for Germany’s ‘victimization’ through the Versailles Treaty. Because the campaign initially gained broad popular and official support, historians have tended to consider the black horror a successful nationalist movement bridging political divides and strengthening the German nation state. In contrast, this essay points to some of the contradictions within the campaign, which often crystallized around conflicts over the nature of effective propaganda. Extreme racist claims about the Rhineland’s alleged ‘mulattoization’ (Mulattisierung) increasingly alienated Rhinelanders and threatened to exacerbate traditional tensions between the predominantly Catholic Rhineland and the central state at a time when Germany’s western borders seemed rather precarious in the light of recent territorial losses and separatist agitation. There was a growing concern that radical strands within the black horror movement were detrimental to the cohesion of the German nation state and to Germany’s positive image abroad, and this was a major reason behind the campaign’s decline after 1921/22. The conflicts within the campaign also point to some hitherto neglected affinities between the black horror and subsequent Nazi propaganda.

Depression is a major public health problem in the Russian Federation and is particularly of concern for men who have sex with men (MSM). MSM living in Moscow City were recruited via respondent-driven sampling and participated in a cross-sectional survey from October 2010 to April 2013. Multiple logistic regression models compared the relationship between sexual identity, recent stigma, and probable depression, defined as a score of ≥23 on the Center for Epidemiological Studies Depression scale. We investigated the interactive effect of stigma and participation in the study after the passage of multiple "anti-gay propaganda laws" in Russian provinces, municipalities, and in neighboring Ukraine on depression among MSM. Among 1367 MSM, 36.7% (n = 505) qualified as probably depressed. Fifty-five percent identified as homosexual (n = 741) and 42.9% identified as bisexual (n = 578). Bisexual identity had a protective association against probable depression (reference: homosexual identity AOR 0.71; 95%CI 0.52-0.97; p < 0.01). Those who experienced recent stigma (last 12 months) were more likely to report probable depression (reference: no stigma; AOR 1.75; 95%CI 1.20-2.56; p < 0.01). The interaction between stigma and the propaganda laws was significant. Among participants with stigma, probable depression increased 1.67-fold after the passage of the anti-gay laws AOR 1.67; 95%CI 1.04-2.68; p < 0.01). Depressive symptoms are common among MSM in Russia and exacerbated by stigma and laws that deny homosexual identities. Repeal of Russia's federal anti-gay propaganda law is urgent but other social interventions may address depression and stigma in the current context.

Traditional university maps, taking campus as the principal body, mainly realize the abilities of space localization and navigation. They don't take full advantage of map, such as multi-scale representations and thematic geo-graphical information visualization. And their inherent propaganda functions have not been entirely developed. Therefore, we tried to take East China Normal University (ECNU) located in Shanghai as an example, and integrated various information related to university propaganda need (like spatial patterns, history and culture, landscape ecology, disciplinary constructions, cooperation, social services, development plans and so on). We adopted the frontier knowledge of `information design' as well as kinds of information graphics and visualization solutions. As a result, we designed and compiled a prototype atlas of `ECNU Impression' to provide a series of views of ECNU, which practiced a new model of `narrative campus map'. This innovative propaganda product serves as a supplement to typical shows with official authority, data maturity, scientificity, dimension diversity, and timing integrity. The university atlas will become a usable media for university overall figure shaping.

Given the increase in the volume of health and medical news over the past few years, the expanding population of journalists committed to feeding the public's voracious appetite for such information, and the important role of government public health organizations in producing and disseminating public health information, it is surprising that little research exists that explores the relationships among public health entities and health journalists. This article describes and analyzes similarities and differences in perceptions between journalists and information officers in public health agencies on a number of issues to reveal how public information officers and health journalists can work to build a local public health agenda free from the burden of unnecessary or inconsistent barriers. This study reports findings from a study with a 3-stage pretest and 90 interviews with state and local public health information officers and the health journalists who cover public health beats across the United States. Despite some agreement, results indicate wide disparities between these populations' identification of what the barriers to high-quality health care and information are, and a generalized absence of a "shared vision."

Historical assessments of the Occupation’s efforts to tackle enteric diseases (cholera, typhoid, paratyphoid and dysentery) have generally reflected a celebratory narrative of US-inspired public health reforms, strongly associated with the head of the Public Health and Welfare Section, Crawford F. Sams. Close inspection of the documentary record, however, reveals much greater continuity with pre-war Japanese public health practices than has hitherto been acknowledged. Indeed, there are strong grounds for disputing American claims of novelty and innovation in such areas as immunisation, particularly in relation to typhoid vaccine, and environmental sanitation, where disparaging comments about the careless use of night soil and a reluctance to control flies and other disease vectors reveal more about the politics of public health reform than the reality of pre-war practices. Likewise, the representation of American-inspired sanitary teams as clearly distinct from and far superior to traditional sanitary associations (eisei kumiai) was closer to propaganda than an accurate rendering of past and present developments. Unfortunately, efforts to acquire, render, and visualize the diversity of health intelligence information are hindered by its wide distribution across disparate fields, multiple levels of government, and the complex interagency environment. Achieving this new level of situation awareness within public health will require a fundamental cultural shift in methods of acquiring, analyzing, and disseminating information. The notion of information "fusion" may provide opportunities to expand data access, analysis, and information exchange to better inform public health action.

In late 1949 the former Soviet Union conducted an open trial of eight Japanese physicians and researchers and four other military servicemen in Khabarovsk, a city in eastern Siberia. Despite its strong ideological tone and many obvious shortcomings such as the lack of international participation, the trial established beyond a reasonable doubt that the Japanese army had prepared and deployed bacteriological weapons and that Japanese researchers had conducted cruel experiments on living human beings. However, the trial, together with the evidence presented to the court and its major findings--which have proved remarkably accurate--was dismissed as communist propaganda and totally ignored in the West until the 1980s. This paper reviews the 1949 Khabarovsk trial, examines the West's dismissal of the proceedings as mere propaganda and draws some moral lessons for bioethics today. As an important historical case, set in the unique socio-political context of the Cold War, the West's dismissal of the trial powerfully illustrates some perennial ethical issues such as the ambivalence of evidence and power of ideology in making (or failing to make) cross-national and cross-cultural factual and moral judgments.

We as Health Physicists face what, at many times, appears to be a hopeless task. The task simply stated is informing the public about the risks (or lack thereof) of radiation. Unfortunately, the public has perceived radiation risks to be much greater than they actually are. An example of this problem is shown in a paper by Arthur C. Upton. Three groups of people -- the League of Women Voters, students, and Business and Professional Club members -- were asked to rank 30 sources of risk according to their contribution to the number of deaths in the United States. Notmore » surprisingly, they ranked nuclear power much higher and medical x-rays much lower than the actual values. In addition to the perception problem, we are faced with another hurdle: health physicists as communicators. Members of the Health Physics Society (HPS) found that the communication styles of most health physicists appear to be dissimilar to those of the general public. These authors administered the Myers-Briggs Type Indicator to the HPS Baltimore-Washington Chapter. This test, a standardized test for psychological type developed by Isabel Myers, ask questions that provide a quantitative measure of our natural preferences in four areas. Assume that you as a health physicist have the necessary skills to communicate information about radiation to the public. Health physicists do nothing with these tools. Most people involved in radiation protection do not get involved with public information activies. What I will attempt to do is heighten your interest in such activities. [The literary impact of the propaganda campaign against veneral disease at the turn of the century].

The literary perception of venereal diseases in the XIXth century was radically modified by the big prophylactic campaign that began at its end. After writers presented a romantic vision of syphilis associated initially with pride and exaltation, came a generation distressed by the obsession and the phobia that the antivenereal reaction then generated. The moving elements of this campaign, were the extreme attention to overvalued statistics and excessive consideration of indirect transmission; but also such mythical concepts as parasyphilis, syphilitic diathesis, "le génie syphilitique" and especially "hérédosyphilis". This antivenereal campaign, appearing as a sanitary prophylaxis invented by the syphiligraphes, rapidly changed to a moral prophylaxis, using intensively dissuasive methods and generating among others a true propaganda literature. After World War I, the fear of a degeneration of the race, weakened by depopulation, caused an intensification of the propaganda. Its protectionist, xenophobic and intolerant nature then grew considerably in the militant literature to merge into the themes that have characterized the political speech of the Hitlerian period. The Allies and penicillin fortunately put an end to this delirious rhetoric.

We propose a general nonlinear analytical framework to study the effect of an external stimulus in the internal state of a population of moving particles. This novel scheme allows us to study a broad range of excitation transport phenomena. In particular, considering social systems, it gives insight of the spatial dynamics influence in the competition between propaganda (mass media) and convincement. By extending the framework presented by Terranova et al. [Europhys. Lett. 105, 30007 (2014), 10.1209/0295-5075/105/30007], we now allow changes in individual's opinions due to a reflection induced by mass media. The equations of the model could be solved numerically, and, for some special cases, it is possible to derive analytical solutions for the steady states. We implement computational simulations for different social and dynamical systems to check the accuracy of our scheme and to study a broader variety of scenarios. In particular, we compare the numerical outcome with the analytical results for two possible real cases, finding a good agreement. From the results, we observe that mass media dominates the opinion state in slow dynamics communities; whereas, for higher agent active speeds, the rate of interactions increases and the opinion state is determined by a competition between propaganda and persuasion. This difference suggests that kinetics can not be neglected in the study of transport of any excitation over a particle system.

Information Seeking When Problem Solving: Perspectives of Public Health Professionals.

Given the many different types of professionals working in public health and their diverse roles, it is likely that their information needs, information-seeking behaviors, and problem-solving abilities differ. Although public health professionals often work in interdisciplinary teams, few studies have explored their information needs and behaviors within the context of teamwork. This study explored the relationship between Canadian public health professionals' perceptions of their problem-solving abilities and their information-seeking behaviors with a specific focus on the use of evidence in practice settings. It also explored their perceptions of collaborative information seeking and the work contexts in which they sought information. Key Canadian contacts at public health organizations helped recruit study participants through their list-servs. An electronic survey was used to gather data about (a) individual information-seeking behaviors, (b) collaborative information-seeking behaviors, (c) use of evidence in practice environments, (d) perceived problem-solving abilities, and (e) demographic characteristics. Fifty-eight public health professionals were recruited, with different roles and representing most Canadian provinces and one territory. A significant relationship was found between perceived problem-solving abilities and collaborative information-seeking behavior (r = -.44, p < .00, N = 58), but not individual information seeking. The results suggested that when public health professionals take a shared, active approach to problem solving, maintain personal control, and have confidence, they are more likely collaborate with others in seeking information to complete a work task. Administrators of public health organizations should promote collaboration by implementing effective communication and information-seeking strategies, and by providing information resources and retrieval tools. 'Never heard of it'- understanding the public's lack of awareness of a new electronic patient record.

The introduction of electronic patient records that are accessible by multiple providers raises security issues and requires informed consent - or at the very least, an opportunity to opt out. Introduction of the Summary Care Record (SCR) (a centrally stored electronic summary of a patient's medical record) in pilot sites in the UK was associated with low awareness, despite an intensive public information programme that included letters, posters, leaflets, and road shows. To understand why the public information programme had limited impact and to learn lessons for future programmes. Linguistic and communications analysis of components of the programme, contextualized within a wider mixed-method case study of the introduction of the SCR in pilot sites. Theoretical insights from linguistics and communication studies were applied. The context of the SCR pilots and the linked information programme created inherent challenges which were partially but not fully overcome by the efforts of campaigners. Much effort was put into designing the content of a mail merge letter, but less attention was given to its novelty, linguistic style, and rhetorical appeal. Many recipients viewed this letter as junk mail or propaganda and discarded it unread. Other components of the information programme were characterized by low visibility, partly because only restricted areas were participating in the pilot. Relatively little use was made of interpersonal communication channels. Despite ethical and legal imperatives, informed consent for the introduction of shared electronic records may be difficult to achieve through public information campaigns. Success may be more likely if established principles of effective mass and interpersonal communication are applied. © 2010 Blackwell Publishing Ltd.

The quantum principles of superposition and entanglement have led to a recasting of the foundations of information and computation theory, and are especially helpful in understanding the nature of privacy. The most private information, exemplified by a quantum eraser experiment, is best regarded as existing only conditionally and temporarily-after the experiment is over no trace remains. Less private is classically-secret information-quantum information that has decohered, and thus become recoverable in principle, though not in practice, from portions of the environment. Finally there is public information, which has been replicated so thoroughly throughout the environment as to be infeasible to conceal. The Internet has caused an explosion of public information, with the beneficial side effect of making it harder for despots to rewrite the history of their misdeeds, and it is tempting to hope that all macroscopic information is permanent, making such cover-ups impossible in principle if not in practice. However, by comparing entropy flows into and out of the Earth with estimates of the planet's storage capacity, we conclude that most macroscopic information-for example the pattern of sand grains on an ancient beach-is impermanent, in the sense of becoming irrecoverable in principle from the Earth though still recorded in the Universe.

Tools to support evidence-informed public health decision making

Background Public health professionals are increasingly expected to engage in evidence-informed decision making to inform practice and policy decisions. Evidence-informed decision making involves the use of research evidence along with expertise, existing public health resources, knowledge about community health issues, the local context and community, and the political climate. The National Collaborating Centre for Methods and Tools has identified a seven step process for evidence-informed decision making. Tools have been developed to support public health professionals as they work through each of these steps. This paper provides an overview of tools used in three Canadian public health departments involved in a study to develop capacity for evidence-informed decision making. Methods As part of a knowledge translation and exchange intervention, a Knowledge Broker worked with public health professionals to identify and apply tools for use with each of the steps of evidence-informed decision making. The Knowledge Broker maintained a reflective journal and interviews were conducted with a purposive sample of decision makers and public health professionals. This paper presents qualitative analysis of the perceived usefulness and usability of the tools. Results Tools were used in the health departments to assist in: question identification and clarification; searching for the best available research evidence; assessing the research evidence for quality through critical appraisal; deciphering the ‘actionable message(s)’ from the research evidence; tailoring messages to the local context to ensure their relevance and suitability; deciding whether and planning how to implement research evidence in the local context; and evaluating the effectiveness of implementation efforts. The methodology for an information campaign is discussed, detailing follow up in five stages: (1) assessment of needs: a good knowledge of behaviour patterns and the kind of information needed is the basic element to create the concept of a campaign. A pre-survey is an indispensable tool to secure this information; (2) basic scientific information: communication to the public has to be based on precise and recent scientific data; (3) comprehension of the information: communicating with the general public should be through texts that are clear and accessible to the greater part of the population; (4) production of supporting documents: each element of the campaign has to produce an impact on the person receiving the message; and (5) evaluating the impact of an information campaign. An evaluation can check whether objectives have been reached, and also correct errors and help prepare the next campaign even more professionally. These five stages are essential to set up a successful information campaign. We also give examples from our magazines Cancer-info and Smoke Buster-info. Through information public opinion can be mobilized to influence political decisions in the public health sector. One evidence for the importance of collective influence on political decision makers is our campaign to promote the total ban on tobacco advertising. As large-scale information campaigns need important financial resources it is essential to balance the cost to generate sufficient funds to finance further projects in the health sector; we discuss this matter as well as the impact on the image of the association.(ABSTRACT TRUNCATED AT 250 WORDS)

Public disclosure of information about the quality of health plans, hospitals, and doctors continues to be controversial. The US experience of the past decade suggests that sophisticated quality measures and reporting systems that disclose information on quality have improved the process and outcomes of care in limited ways in some settings, but these efforts have not led to the "consumer choice" market envisaged. Important reasons for this failure include limited salience of objective measures to consumers, the complexity of the task of interpretation, and insufficient use of quality results by organised purchasers and insurers to inform contracting and pricing decisions. Nevertheless, public disclosure may motivate quality managers and providers to undertake changes that improve the delivery of care. Efforts to measure and report information about quality should remain public, but may be most effective if they are targeted to the needs of institutional and individual providers of care. As part of the National Information Infrastructure, the U.S. federal government is establishing a Government Information Locator Service (GILS). GILS will identify and describe public information resources throughout the federal government and provide assistance in obtaining the information. It will be decentralized and will supplement other agency and commercial information dissemination mechanisms. The public will use GILS directly or through intermediaries, including the Government Printing Office and the National Technical Information Service, as well as federal depository libraries, other public libraries, and private sector information services. Direct users will have access to a GILS Core accessible on the Internet without charge. Intermediate access may include kiosks, 800 numbers, electronic mail, bulletin boards, FAX, and offline media such as floppy disks, CD-ROM, and printed works. GILS will use network technology and the American National Standards Institute Z39.50 standard for information search and retrieval so that information can be retrieved in a variety of ways. Direct users may have access to many other major federal and nonfederal information resources, linkages to data systems, and electronic delivery of information products. An Office of Management and Budget Bulletin in 1994 will provide implementing guidance to agencies. The 2009 Family Smoking Prevention and Tobacco Control Act (TCA) requires tobacco companies to disclose information about the harmful chemicals in their products to the U.S. Food and Drug Administration (FDA). The law requires the FDA, in turn, to communicate this information to the public "in a format that is understandable and not misleading to a lay person." But how should the FDA comply with this requirement? What does it mean for information about complex chemicals to be "understandable and not misleading to a lay person"? These questions are not easy ones to answer. Disclosures about the amount of harmful chemicals (constituents) in different tobacco products may help to inform consumers, but may also conversely prompt consumers to reach incorrect or unsupported conclusions about products' relative health risks. This paper first analyzes the FDA's legal obligation to publish tobacco constituent information so that it is "understandable and not misleading to a layperson." Second, it discusses how that legal analysis has guided scientific research examining how members of the public interpret messages regarding tobacco constituents. Lastly, this paper concludes with policy recommendations for the FDA as it considers how to comply with the law's constituent disclosure requirement while still furthering its overall objective of promoting public health.

Behavior analysts have become increasingly concerned about inaccuracies and misconceptions in the public, educational, and professional information portraying their activities, but have done little to correct these views. The present paper has two purposes in this regard. First, the paper describes some of the conditions that have given rise to these concerns. Second, and more important, the paper surveys various procedures and programs for the dissemination of public information that may correct inaccuracies and misconceptions. Special consideration is also given to issues involving (a) the assessment of the problem, (b) the content and means of dissemination, (c) the possible contributions of behavior analysts to current misunderstandings, and (d) relationships between behavior analysts and the media. The dissemination of accurate and unbiased information constitutes an important new undertaking for behavior analysis. The future of the field may depend in part on such activity. PMID:22478623

Distributed Data Networks That Support Public Health Information Needs.

Data networks, consisting of pooled electronic health data assets from health care providers serving different patient populations, promote data sharing, population and disease monitoring, and methods to assess interventions. Better understanding of data networks, and their capacity to support public health objectives, will help foster partnerships, expand resources, and grow learning health systems. We conducted semistructured interviews with 16 key informants across the United States, identified as network stakeholders based on their respective experience in advancing health information technology and network functionality. Key informants were asked about their experience with and infrastructure used to develop data networks, including each network's utility to identify and characterize populations, usage, and sustainability. Among 11 identified data networks representing hundreds of thousands of patients, key informants described aggregated health care clinical data contributing to population health measures. Key informant interview responses were thematically grouped to illustrate how networks support public health, including (1) infrastructure and information sharing; (2) population health measures; and (3) network sustainability. Collaboration between clinical data networks and public health entities presents an opportunity to leverage infrastructure investments to support public health. Data networks can provide resources to enhance population health information and infrastructure.

The UK's leading professional body for public relations "Chartered Institute of Public Relations" (CIPR) said that the public relations is about reputation--they are the result of what you do, what you say and what others say about you. Furthermore CIPR says that public relations are discipline whose objectives are safeguarding reputation, establishing understanding and pot pores, and the impact on the thinking and behavior of the public. Although the primary goal of public relations is to preserve and build a reputation, to tell the truth to a customer who has hired experts in this area, it seems that in its own way of development, public relations practitioners have stopped worrying about their reputation and the perception of the discipline within the public they address. All relevant professional bodies for public relations, including the Croatian Association for Public Relation (HUOJ), had set up codes of ethics and high standards according which the members and practitioners should be evaluated. Among other things stays that practitioner of public relations is required to check the reliability and accuracy of the data prior to their distribution and nurture honesty and accountability to the public interest. It seems that right this instruction of code of ethics has been often violated. In a public speech in Croatia, and therefore in the media, exist manipulation, propaganda, and all the techniques of spin, which practitioners of public relations are skillfully using in the daily transfer of information to the users and target groups. The aim of this paper is to determine what is the perception of the profession to the public. As in today's journalism increasingly present plume of public relations, we wish to comment on the part where journalism ends and begins PR and vice versa. Factors that influence public awareness of domestic waste characteristics and management in rural areas.

Public awareness of domestic waste characteristics and management (PADWCM) is a prerequisite for a domestic waste management (DWM) plan. In this study, we considered China as a typical example of developing countries to investigate the public perception of environmental pollution (PEP), its PADWCM, and its socioeconomic factors, using questionnaires and statistical methods. The results indicated that the public PEP was mainly due to obvious environmental contamination in local villages, and the PADWCM is still inadequate in rural areas. However, 83.9% of the respondents agreed that DWM is highly necessary. The PADWCM, including the hazard, recyclability, and characteristic pollution caused by domestic waste, was learned mainly from people's direct PEP, experiences, and repurchasing by recyclers. Education period had highly significant positive correlations and regressions with PEP and PADWCM, respectively. Additionally, education directly affected public awareness in different income and age groups. Demonstration projects had a significant positive effect on the awareness of characteristic pollution caused by domestic waste, which was readily perceived. Propaganda had a significant positive influence on the awareness of treatment necessity. Age and gender had no obvious effects on public PEP and PADWCM. Some aspects of PADWCM requiring a deep understanding became stronger as income increased; however, the PADWCM located close to poor people became weaker. Some aspects of PEP could significantly influence the awareness of characteristic pollution caused by domestic waste and awareness of treatment necessity. Therefore, improving environmental education, performing demonstration projects and effective environmental propaganda, increasing people's income, and attracting young people to participate in DWM will be important for enhancing environmental awareness and DWM in rural areas. The National Plan to Address Alzheimer's Disease calls for a new relationship between researchers and members of the public. This relationship is one that provides research information to patients and allows patients to provide ideas to researchers. One way to describe it is a "bidirectional translational relationship." Despite the numerous sources of online and offline information about Alzheimer's disease, there is no information source which currently provides this interaction. This article proposes the creation an Alzheimer's research information source dedicated to monitoring Alzheimer's research literature and providing user friendly, publicly accessible summaries of data written specifically for a lay audience. This information source should contain comprehensive, updated, user friendly, publicly available, reviews of Alzheimer's research and utilize existing online multimedia/social networking tools to provide information in useful formats that help patients, caregivers, and researchers learn rapidly from one another.

Social foraging with partial (public) information.

Group foragers can utilize public information to better estimate patch quality and arrive at more efficient patch-departure rules. However, acquiring such information may come at a cost; e.g. reduced search efficiency. We present a Bayesian group-foraging model in which social foragers do not require full awareness of their companions' foraging success; only of their number. In our model, patch departure is based on direct estimates of the number of remaining items. This is achieved by considering all likely combinations of initial patch-quality and group foraging-success; given the individual forager's experience within the patch. Slower rates of information-acquisition by our 'partially-aware' foragers lead them to over-utilize poor patches; more than fully-aware foragers. However, our model suggests that the ensuing loss in long-term intake-rates can be matched by a relatively low cost to the acquisition of full public information. In other words, we suggest that group-size offers sufficient information for optimal patch utilization by social foragers. We suggest, also, that our model is applicable to other situations where resources undergo 'background depletion', which is coincident but independent of the consumer's own utilization. Interplay between social debate and propaganda in an opinion formation model

We introduce a simple model of opinion dynamics in which a two-state agent modified Sznajd model evolves due to the simultaneous action of stochastic driving and a periodic signal. The stochastic effect mimics a social temperature, so the agents may adopt decisions in support for or against some opinion or position, according to a modified Sznajd rule with a varying probability. The external force represents a simplified picture by which society feels the influence of the external effects of propaganda. By means of Monte Carlo simulations we have shown the dynamical interplay between the social condition or mood and the external influence, finding a stochastic resonance-like phenomenon when we depict the noise-to-signal ratio as a function of the social temperature. In addition, we have also studied the effects of the system size and the external signal strength on the opinion formation dynamics.

As European colonization spread widely over the African continent the health and physical welfare of the African population gained more and more importance to European colonists who concentrated on capitalizing on African human resources for an improved financial and economic outcome of their colonies. This brought tropical medicine to the top of the European colonial agenda and raised the awareness of the threat of infectious diseases, such as the African Trypanosomiasis or so-called sleeping disease. In 1916 a group of scientists from the pharmaceutical company Bayer AG discovered a substance on the base of dye rather than arsenic. The drug was called Bayer 205 and showed outstanding therapeutic effects. It also reduced adverse reactions in people infected with sleeping disease. As Germany had already lost its colonies, the Bayer company--supported by the German government--negotiated with the English and Belgian governments and was allowed to send an expedition to East Africa. During 1921 and 1923 the new drug was tested in English Rhodesia and Belgian Congo and proved revolutionary, especially in comparison with conventional substances. In due course, the drug Bayer 205 was named Germanin and it was subsequently proposed to use it for political leverage: knowledge and use of the new drug was to be given only in exchange for parts of the former German colonies. However, the reactions of the international media put an end to Germany's neo-colonial-dreams, even before the proposal had reached governmental level. Even so, the incident never disappeared from the mind of those who wished to revive German colonialism. Thus, it is no surprise, that the tale of the discovery and perceived "injustice" of a thwarted scientific success regained an important place in National Socialist propaganda. Evidence-based information needs of public health workers: a systematized review.

This study assessed public health workers' evidence-based information needs, based on a review of the literature using a systematic search strategy. This study is based on a thesis project conducted as part of the author's master's in public health coursework and is considered a systematized review. Four databases were searched for English-language articles published between 2005 and 2015: PubMed, Web of Science, Library Literature & Information Science Index, and Library, Information Science & Technology Abstracts (LISTA). Studies were excluded if there was no primary data collection, the population in the study was not identified as public health workers, "information" was not defined according to specific criteria, or evidence-based information and public health workers were not the major focus. Studies included in the final analysis underwent data extraction, critical appraisal using CASP and STROBE checklists, and thematic analysis. Thirty-three research studies were identified in the search, including twenty-one using quantitative methods and twelve using qualitative methods. Critical appraisal revealed many potential biases, particularly in the validity of research. Thematic analysis revealed five common themes: (1) definition of information needs, (2) current information-seeking behavior and use, (3) definition of evidence-based information, (4) barriers to information needs, and (5) public health-specific issues. Recommendations are given for how librarians can increase the use of evidence-based information in public health research, practice, and policy making. Further research using rigorous methodologies and transparent reporting practices in a wider variety of settings is needed to further evaluate public health workers' information needs.

Public sector support is essential in 5 main aspects of child health information technology, namely, data standards, pediatric functions in health information systems, privacy policies, research and implementation funding, and incentives for technology adoption. Some innovations in health information technology for adult populations can be transferred to or adapted for children, but there also are unique needs in the pediatric population. Development of health information technology that addresses children's needs and effective adoption of that technology are critical for US children to receive care of the highest possible quality in the future.

The more informed the public is, the better actions they are able to make, resulting in reduced disaster impacts. The criteria for what information to provide the public needs to change depending on the specific needs of the disaster affected population. The method of dissemination also needs to match the communication channels that the public typically uses in disaster situations. This research project investigates the dynamic information needs of disaster affected populations and how information leads to actions. The purpose of the research project is to identify key indicators for measuring how well informed the public is during disasters. The indicators are limited to those which can be observed as communication is happening (i.e., in near-real-time). By doing so, the indicators can be analyzed as disaster situations unfold, deficiencies can be identified, and recommendations can be made to potentially improve communication while the response is still underway. The end goal of the research is to improve the ability of communicators to inform disaster affected communities. A classification scheme has been developed to categorize the information provided to the public during disasters. Under each category is a set of typical questions that the information should answer. These questions are the result of a best observed practice review of the information available during 11 disasters. For example, under the category 'Life Saving Response', the questions which should be answered are who is doing what (Evacuation, SAR), where and when, and the amount of the affected communities' needs being covered by these actions. Protecting public health requires the acquisition, use, and storage of extensive health-related information about individuals. The electronic accumulation and exchange of personal data promises significant public health benefits but also threatens individual privacy; breaches of privacy can lead to individual discrimination in employment, insurance, and government programs. Individuals concerned about privacy invasions may avoid clinical or public health tests, treatments, or research. Although individual privacy protections are critical, comprehensive federal privacy protections do not adequately protect public health data, and existing state privacy laws are inconsistent and fragmented. The Model State Public Health Privacy Act provides strong privacy safeguards for public health data while preserving the ability of state and local public health departments to act for the common good.

The discernment of resource quality is pertinent to many daily decisions faced by animals. Public information is a critical information source that promotes quality assessments, attained by monitoring others’ performance. Here we provide the first evidence, to our knowledge, that chimpanzees (Pan troglodytes) use public information to guide resource selection. Thirty-two chimpanzees were presented with two simultaneous video demonstrations depicting a conspecific acquiring resources at a fast (resource-rich) or slow (resource-poor) rate. Subsequently, subjects selected the resource-rich site above chance expectation. As a comparison, we report evidence of public information use in young children. Investigation of public information use in primates is pertinent, as it can enhance foraging success and potentially facilitate payoff-biased social learning. PMID:24060244

Public libraries have been called the “first responders” to the specialized health information needs of the general public. The challenges inherent in consumer health information (CHI) service are centered around the Patron, the Librarian, the Information Resources, and the Library itself. The pilot study involved interviews with nine individual library workers in eight public libraries in four library systems: the District of Columbia, Montgomery and Prince George’s Counties in Maryland, and Fairfax County in Virginia. Library workers were asked about common consumer health information requests, the nature of their collections, and the role of public libraries in meeting these information needs. The subjects were also presented with a hypothetical scenario, and their responses suggest knowledge gaps. The findings point to the increasing necessity and importance of training and support for public librarians, as well as the importance of understanding where the medical knowledge gaps exist. Public librarians need to commit to formal evaluation of their skill sets and knowledge gaps, in order to identify areas to which libraries can devote limited resources. PMID:26550002

The need for rapid access to information to support critical decisions in public health cannot be disputed; however, development of such systems requires an understanding of the actual information needs of public health professionals. This paper reports the results of a literature review focused on the information needs of public health professionals. The authors reviewed the public health literature to answer the following questions: (1) What are the information needs of public health professionals? (2) In what ways are those needs being met? (3) What are the barriers to meeting those needs? (4) What is the role of the Internet in meeting information needs? The review was undertaken in order to develop system requirements to inform the design and development of an interactive digital knowledge management system. The goal of the system is to support the collection, management, and retrieval of public health documents, data, learning objects, and tools. The search method extended beyond traditional information resources, such as bibliographic databases, tables of contents (TOC), and bibliographies, to include information resources public health practitioners routinely use or have need to use--for example, grey literature, government reports, Internet-based publications, and meeting abstracts. Although few formal studies of information needs and information-seeking behaviors of public health professionals have been reported, the literature consistently indicated a critical need for comprehensive, coordinated, and accessible information to meet the needs of the public health workforce. Major barriers to information access include time, resource reliability, trustworthiness/credibility of information, and "information overload". In July 1981, the Planetree Health Resource Center opened on the San Francisco campus of California Pacific Medical Center (Pacific Presbyterian Medical Center). Planetree was founded on the belief that access to information can empower people and help them face health and medical challenges. The Health Resource Center was created to provide medical library and health information resources to the general public. Over the last twelve years, Planetree has tried to develop a consumer health library collection and information service that is responsive to the needs and interests of a diverse public. In an effort to increase accessibility to the medical literature, a consumer health library classification scheme was created for the organization of library materials. Promoting information sharing for multijurisdictional public health emergency preparedness.

The objective was to assess the planning needs of emergency management and public health professionals to provide a flexible and comprehensive planning tool. This study first assessed the needs of emergency management and public health professionals via an online survey. On the basis of results of the assessment, pertinent information was collected and organized into an online resource tool. The assessment was designed to address the needs of local, state, and federal government administrators working in emergency management and public health. The online tool was designed for use by any entity that functions to promote public health in the event of an emergency. Sixty-four participants completed the assessment survey. Seven states were represented. Most participants were senior-level administrators or management-level employees and were employed in public health, emergency, or bioterrorism preparedness, or in emergency medical services. Needs assessment for preparedness tools. The results of the survey identified a need for increased access to information (especially concerning liability issues and authority to enter into agreements) and high levels of interest in the availability of an online planning tool. The majority (80.7%) of respondents indicated an ability to locate and quantify resources within their own jurisdiction but only about half (42.9%) could do the same for resources outside of their jurisdiction. Finally, 71.9% reported having no assessment tool to measure emergency capacity and limitations. Planning for cross-border and multijurisdictional emergencies depends on access to pertinent information and the feasibility of attaining such information. The creation of a comprehensive guide to multijurisdictional collaborations, with its self-assessment checklists, can easily provide such information to emergency. In addition, information sharing and increased collaboration can lead to increased utilization of emergency preparedness best practices.

This article is part of a series written for people responsible for making decisions about health policies and programmes and for those who support these decision makers. In this article, we address strategies to inform and engage the public in policy development and implementation. The importance of engaging the public (both patients and citizens) at all levels of health systems is widely recognised. They are the ultimate recipients of the desirable and undesirable impacts of public policies, and many governments and organisations have acknowledged the value of engaging them in evidence-informed policy development. The potential benefits of doing this include the establishment of policies that include their ideas and address their concerns, the improved implementation of policies, improved health services, and better health. Public engagement can also be viewed as a goal in itself by encouraging participative democracy, public accountability and transparency. We suggest three questions that can be considered with regard to public participation strategies. These are: 1. What strategies can be used when working with the mass media to inform the public about policy development and implementation? 2. What strategies can be used when working with civil society groups to inform and engage them in policy development and implementation? 3. Our objectives were to review and analyze the laws in the 50 states, the District of Columbia, and Puerto Rico that regulate the acquisition, storage, and use of public health data and to offer proposals for reform of the laws on public health information privacy. Virtually all states reported some statutory protection for governmentally maintained health data for public health information in general (49 states), communicable diseases (42 states), and sexually transmitted diseases (43 states). State statutes permitted disclosure of data for statistical purposes (42 states), contact tracing (39 states), epidemiologic investigations (22 states), and subpoena or court order (14 states). The survey revealed significant problems that affect both the development of fair and effective public health information systems and the protection of privacy. Statutes may be silent about the degree of privacy protection afforded, confer weaker privacy protection to certain kinds of information, or grant health officials broad discretion to disseminate personal information. Our proposals for law reform are based on a meeting of experts at the Carter Presidential Center under the auspices of the Centers for Disease Control and Prevention and the Council of State and Territorial Epidemiologists: (1) an independent data protection commission should be established, (2) health authorities should justify the collection of personally identifiable information, (3) subjects should be given basic information about data practices, (4) data should be held and used in accordance with fair information practices, (5) legally binding privacy and security assurances should attach to identifiable health information with significant penalties for breach of these assurances, (6) disclosure of data should be made only for purposes consistent with the original collection, and (7) secondary uses beyond those originally intended by the data collector should be permitted only with informed consent.

In animals, including humans, the social environment can serve as a public information network in which individuals can gather public information about the quality of potential mates by observing conspecifics during sexual interactions. The observing individual itself is also a part of this information network. When recognized by the observed conspecifics as an audience, his/her presence could influence the sexual interaction between those individuals, because the observer might be considered as a potential mate or competitor. One of the most challenging questions in sexual selection to date is how the use of public information in the context of mate choice is linked to the fitness of individuals. Here, we could show that public information influences mate-choice behaviour in sailfin molly males, Poecilia latipinna, and influences the amount of sperm males transfer to a female partner. In the presence of an audience male, males spent less time with the previously preferred, larger of two females and significantly more time with the previously non-preferred, smaller female. When males could physically interact with a female and were faced with an audience male, three audience females or no audience, males transferred significantly more sperm to a female partner in the presence of an audience male than with female audience or no audience and spent less time courting his female partner. This is the first study showing that public information use turns into fitness investment, which is the crucial factor to understand the role of public information in the dynamic processes in sexual selection.

As part of a larger study of the effects of giving patients written take-home information with prescription medications, a "patient package insert" (PPI) for diazepam was prepared based on content determined by "experts." This report compares the experts' judgments of what information should be included with judgments obtained from the public. Information judged to be most important for inclusion in a PPI was identified by having subjects sort cards containing facts about diazepam. Subjects who had previously used diazepam were no different in their judgments than inexperienced subjects. In general, there was a high degree of concordance between public and expert judgments and also a remarkably strong consensus across very different demographic samples. In those few instances of disagreement, the public attached even greater importance to warnings and "bad news" about diazepam than to information providing reassurances, benign general education, and "good news." To what extent patients would effectively use this information--whether conveyed by PPIs or alternative educational routes--must await empirical evaluation.

Just as the natural environment can be polluted, so too can public discourse. I'm Right and You're an Idiot: The Toxic State of Public Discourse and How to Clean it Up examines the sorry state of today's public square, showing how polluted, polarized conversations discourage people from taking action on critical issues such as climate change, and demonstrating how we can clear the air and become more powerful and effective communicators. The most urgent environmental threat today is not climate change, species extinction or the declining health of oceans. Author Jim Hoggan says the more pressing problem is resistance to change that results from pollution in the public square. A smog of adversarial rhetoric, propaganda and polarization that comes from all sides, is stifling discussion and our ability to solve problems collectively. Hoggan has investigated this troubling state of public dialogue and in his new book he asks leading intellectuals why, despite overwhelming scientific evidence, so little is being done to address today's critical environmental challenges. As chair of the David Suzuki Foundation and founder of one of Canada's most successful PR firms, he is uniquely positioned to probe this question. Science Center Public Forums: Engaging Lay-Publics in Resilience Deliberations Through Informal Science Education

Because climate hazards present a range of potential impacts and considerations for different kinds of stakeholders, community responses to increase resilience are best considered through the inclusion of diverse, informed perspectives. The Science Center Public Forums project has created multifaceted modules to engage diverse publics in substantive deliberations around four hazards: heat waves, drought, extreme precipitation, and sea level rise. Using a suite of background materials including visualization and narrative components, each of these daylong dialogues engage varied groups of lay-participants at eight US science centers in learning about hazard vulnerabilities and tradeoffs of proposed strategies for building resilience. Participants listen to and consider the priorities and perspectives of fellow residents and stakeholders, and work together to formulate detailed resilience plans reflecting both current science and informed public values. Deliverables for the project include visualizations of hazard vulnerabilities and strategies through immersive planetarium graphics and Google Earth, stakeholder perspective narratives, and detailed background materials for each project hazard. This session will: communicate the process for developing the hazard modules with input from subject matter experts, outline the process for iterative revisions based upon findings from formative focus groups, share results generated by participants of the project's first two pilot forums, and describe plans for broader implementation. These activities and outcomes could help to increase the capacity of informal science education institutions as trusted conveners for informed community dialogue by educating residents about vulnerabilities and engaging them in critical thinking about potential policy responses to critical climate hazards while sharing usable public values and priorities with civic planners.

Public health, prevention, health education and health promotion are inseparable from the concepts of information and communication. Information should respond as much as possible to the needs of professionals, decision-makers, and consumers who are more and more concerned and conscious of its importance in light of "information overload", various dissemination channels and the multiplicity of its sources. There are numerous issues at stake ranging from comprehension, to the validation of health information, health education, health promotion, prevention, decision-making, as well as issues related to knowledge and power. Irrespective of the type of choice to be made, the need for information, knowledge, and know-how is inseparable from that of other tools or regulatory measures required for decision-making. Information is the same as competence, epidemiological and population data, health data, scientific opinion, and expert conferences--all are needed to assist in decision-making. Based on the principle of precaution, information must increasingly take into account the rejection of a society which often reasons on the basis of a presumption of zero-risk, in an idealistic manner, and which also excludes the possibility of new risks. The consumer positions himself as the regulator of decisions, specifically those with regard to the notion of acceptable level of risk. All of the actors involved in the health system are or become at one moment or another public health decision-makers. Their decision might be based either on an analytical approach, or on an intuitive approach. Although the act of decision-making is the least visible part of public health policy, it is certainly the driving force. This process should integrate the perspective of all of the relevant players, including consumers, who are currently situated more and more frequently at the heart of the health system. Public attitudes to genomic science: an experiment in information provision.

We use an experimental panel study design to investigate the effect of providing "value-neutral" information about genomic science in the form of a short film to a random sample of the British public. We find little evidence of attitude change as a function of information provision. However, our results show that information provision significantly increased dropout from the study amongst less educated respondents. Our findings have implications both for our understanding of the knowledge-attitude relationship in public opinion toward genomic science and for science communication more generally.

Media facades (often characterized as a building's digital skin) are public displays that substitute dynamic details and information for usually static structures. SMSlingshot is a media facade system at the confluence of art, architecture, and technology design in the context of urban human-computer interaction. It represents a participative approach to public displays that enlivens public spaces and fosters civic and social dialogue as an alternative to advertising and service-oriented information displays. Observations from SMSlingshot's implementation at festival exhibitions provide insight into the roles of scale, distance, and the spatial situation of media facade contexts. The lessons learned apply to most public-display situations and will be useful for designers and developers of this new medium in urban spaces.

A Revolutionary Attack on Tobacco: Bolshevik Antismoking Campaigns in the 1920s

Using archival records of the Commissariat of Public Health, journals, and propaganda materials from the antismoking campaign of the Soviet 1920s, this article argues that the revolutionary state pursued an antitobacco policy unique in the world in its attack on tobacco use at a national scale. The commissar of public health, Nikolai Alexandrovich Semashko, attempted to severely curtail tobacco cultivation and production, limit tobacco sales, and create a public opinion against tobacco with a propaganda campaign. Even in failing in its farther-reaching goals, the policy proved one of the most forward in terms of antismoking propaganda and state-sponsored treatment regimens, with the distribution of antismoking posters, pamphlets, articles, plays, and films as well as the creation of special state-sponsored smoking-cessation programs that boasted high success rates. Public reporting of cost and quality information in orthopaedics.

Public reporting of patient health outcomes offers the potential to incentivize quality improvement by fostering increased accountability among providers. Voluntary reporting of risk-adjusted outcomes in cardiac surgery, for example, is viewed as a "watershed event" in healthcare accountability. However, public reporting of outcomes, cost, and quality information in orthopaedic surgery remains limited by comparison, attributable in part to the lack of standard assessment methods and metrics, provider fear of inadequate adjustment of health outcomes for patient characteristics (risk adjustment), and historically weak market demand for this type of information. We review the origins of public reporting of outcomes in surgical care, identify existing initiatives specific to orthopaedics, outline the challenges and opportunities, and propose recommendations for public reporting of orthopaedic outcomes. We performed a comprehensive review of the literature through a bibliographic search of MEDLINE and Google Scholar databases from January 1990 to December 2010 to identify articles related to public reporting of surgical outcomes. Orthopaedic-specific quality reporting efforts include the early FDA adverse event reporting MedWatch program and the involvement of surgeons in the Physician Quality Reporting Initiative. Issues that require more work include balancing different stakeholder perspectives on quality reporting measures and methods, defining accountability and attribution for outcomes, and appropriately risk-adjusting outcomes. Given the current limitations associated with public reporting of quality and cost in orthopaedic surgery, valuable contributions can be made in developing specialty-specific evidence-based performance measures. We believe through leadership and involvement in policy formulation and development, orthopaedic surgeons are best equipped to accurately and comprehensively inform the quality reporting process and its application to improve the

Public information and communication technologies, such as information kiosks, automated banking machines and ticket dispensers, allow people to access services in a convenient and timely manner. However, the development of these technologies has occurred largely without consideration of access by people with disabilities. Inaccessible technical features make operation of a public technology difficult and barriers in the environment create navigational challenges, limiting the opportunity of people with disabilities to use these devices and access the services they provide. This paper describes the development of a tool that individuals, disability advocacy groups, business owners, healthcare providers, and urban planners can use to evaluate the accessibility of public technologies and the surrounding environment. Evaluation results can then be used to develop recommendations and advocate for technical and environmental changes to improve access. Tool development consisted of a review of the literature and key Canadian Standards Association documents, task analysis, and consultation with accessibility experts. Studies of content validity, tool usability, inter-rater and test-retest reliability were conducted in sites across Canada. Accessibility experts verified the content validity of the tool. The current version of the tool has incorporated the findings of a usability study. Initial testing indicated excellent agreement for inter-rater and test-retest reliability scores. Social exclusion can arise when public technologies are not accessible. This newly developed instrument provides detailed information that can be used to advocate for more accessible and inclusive public information and communication technologies.

Unlike incentives in the clinical care system, scant funding is available to public health departments to develop the necessary information infrastructure and workforce capacity to capitalize on EHRs, personal health records, or Big Data. Current EHR systems are primarily built to serve clinical systems and practice rather than being structured for public health use. In addition, there are policy issues concerning how broadly the data can be used by public health officials. An article, “A Meta-analytic Examination of Assumed Properties of Child Sexual Abuse Using College Samples,” published in the July 1998 edition of the Psychological Bulletin resulted in an unprecedented amount of media attention and became the first scientific article to be formally denounced by the United States House of Representatives. The study's authors analyzed the findings of 59 earlier studies on child sexual abuse (CSA) and concluded that mental health researchers have greatly overstated CSA's harmful potential. They recommended that a willing encounter with positive reactions would no longer be considered to be sexual abuse; instead, it would simply be labeled adult-child sex. The study's conclusions and recommendations spawned a debate in both the popular and scholarly press. A number of commentators suggested that the study is pedophile propaganda masquerading as science. Others claimed that the authors are victims of a moralistic witch-hunt and that scientific freedom is being threatened. After a careful examination of the evidence, it is concluded that Rind et al. can best be described as an advocacy article that inappropriately uses science in an attempt to legitimize its findings.

Beyond information access: Support for complex cognitive activities in public health informatics tools.

Public health professionals work with a variety of information sources to carry out their everyday activities. In recent years, interactive computational tools have become deeply embedded in such activities. Unlike the early days of computational tool use, the potential of tools nowadays is not limited to simply providing access to information; rather, they can act as powerful mediators of human-information discourse, enabling rich interaction with public health information. If public health informatics tools are designed and used properly, they can facilitate, enhance, and support the performance of complex cognitive activities that are essential to public health informatics, such as problem solving, forecasting, sense-making, and planning. However, the effective design and evaluation of public health informatics tools requires an understanding of the cognitive and perceptual issues pertaining to how humans work and think with information to perform such activities. This paper draws on research that has examined some of the relevant issues, including interaction design, complex cognition, and visual representations, to offer some human-centered design and evaluation considerations for public health informatics tools.

GPs' opinions of public and industrial information regarding drugs: a cross-sectional study

Background General Practitioners {GP} in Sweden prescribe more than 50% of all prescriptions. Scientific knowledge on the opinions of GPs regarding drug information has been sparse. Such knowledge could be valuable when designing evidence-based drug information to GPs. GPs' opinions on public- and industry-provided drug information are presented in this article. Methods A cross-sectional study using a questionnaire was answered by 368 GPs at 97 primary-health care centres {PHCC}. The centres were invited to participate by eight out of 29 drug and therapeutic committees {DTCs}. A multilevel model was used to analyse associations between opinions of GPs regarding drug information and whether the GPs worked in public sector or in a private enterprise, their age, sex, and work experience. PHCC and geographical area were included as random effects. Results About 85% of the GPs perceived they received too much information from the industry, that the quality of public information was high and useful, and that the main task of public authorities was to increase the GPs' knowledge of drugs. Female GPs valued information from public authorities to a much greater extent than male GPs. Out of the GPs, 93% considered the main task of the industry was to promote sales. Differences between the GPs' opinions between PHCCs were generally more visible than differences between areas. Conclusions Some kind of incentives could be considered for PHCCs that actively reduce drug promotion from the industry. That female GPs valued information from public authorities to a much greater extent than male GPs should be taken into consideration when designing evidence-based drug information from public authorities to make implementation easier. Socially acquired information improves the accuracy and efficiency of environmental assessments and can increase fitness. Public information may be especially useful during unpredictable food conditions, or for species that depend on resources made less predictable by human disturbance. However, the physiological mechanisms by which direct foraging assessments and public information are integrated to affect behaviour remain largely unknown. We tested for potential effects of public information on the behavioural and hormonal response to food reduction by manipulating the social environment of captive red crossbills (Loxia curvirostra). Red crossbills are irruptive migrants that are considered sensitive to changes in food availability and use public information in decision making. Here, we show that public information can attenuate or intensify the release of glucocorticoids (i.e. stress hormones) during food shortage in red crossbills. The observed modulation of corticosterone may therefore be a physiological mechanism linking public information, direct environmental assessments and behavioural change. This mechanism would not only allow for public information to affect individual behaviour, but might also facilitate group decision making by bringing group members into more similar physiological states. The results further suggest that stressors affecting entire populations may be magnified in individual physiology through social interactions. PMID:20356895

In the old Soviet Union there was a periodical called The Propagandist's Notepad devoted to those whose path to happiness lay in "getting the message across". The demise of the socialist model of wealth creation brought an end to this magazine. In many ways, however, Liz Kalaugher's opinion article "Communicating climate change" (January pp16-17) is a tribute to the need for such a specialized publication today. The article describes ways in which climate scientists can improve how they transmit their message to the public. Unfortunately, physicists tend to be guided by Descartes's ubiquitous principle of the virtue of doubt, and are inclined to regard "the public becoming increasingly sceptical" as commendable. Indeed, I wish there were more examples of public scepticism.

Multi-coil (MC) magnetic field modeling has emerged as a viable alternative to conventional field generation based on spherical harmonic shapes, and an active MC community is forming. Although all MC applications share the same modeling concept, the specific MC designs can largely differ as a result of disparities in region of interest (eg, human versus rodent), intended MR application (eg, B 0 shimming versus spatial encoding), or other experimental constraints (eg, available bore space or integration with radiofrequency technology). To date, a lack of detailed information on existing MC designs complicates the assessment and precludes a meaningful comparison. Here, we suggest that future publications involving the MC technique not only report the benefits for the application at hand, but also include an explicit description of the MC wire pattern used. This public multi-coil information (PUMCIN) policy represents a voluntary commitment to promoting free public access to the details necessary for reproducing and benefiting from MC research. The PUMCIN policy is expected to initiate a paradigm shift with respect to the way MC innovation is reported. By setting an example, we hope to encourage the evolving MC community to maximize the benefits for science and society by embracing it. The National Institutes of Health and other federal health agencies are considering establishing a national biobank to study the roles of genes and environment in health. We assessed the public's attitudes toward the proposed biobank, including preferences for providing informed consent. Sixteen focus groups were conducted, and themes arising from the focus groups were tested in a large, representative survey (n = 4659) of the general population. Our research demonstrates that when considering participating in a genomic biobank, individuals want ongoing choices and control over access to their samples and information. PMID:19833988

Stimulating Contributions to Public Goods through Information Feedback: Some Experimental Results

In traditional public good experiments participants receive an endowment from the experimenter that can be invested in a public good or kept in a private account. In this paper we present an experimental environment where participants can invest time during five days to contribute to a public good. Participants can make contributions to a linear public good by logging into a web application and performing virtual actions. We compared four treatments, with different group sizes and information of (relative) performance of other groups. We find that information feedback about performance of other groups has a small positive effect if we control for various attributes of the groups. Moreover, we find a significant effect of the contributions of others in the group in the previous day on the number of points earned in the current day. Our results confirm that people participate more when participants in their group participate more, and are influenced by information about the relative performance of other groups. PMID:27459070

Withholding differential risk information on legal consumer nicotine/tobacco products: The public health ethics of health information quarantines.

The United States provides an example of a country with (a) legal tobacco/nicotine products (e.g., snus, other smokeless tobacco, cigarettes) differing greatly in risks to health and (b) respected health information websites that continue to omit or provide incorrect differential risk information. Concern for the principles of individual rights, health literacy, and personal autonomy (making decisions for oneself), which are key principles of public health ethics, has been countered by utilitarian arguments for the use of misleading or limited information to protect public health overall. We argue that omitting key health relevant information for current or prospective consumers represents a kind of quarantine of health-relevant information. As with disease quarantines, the coercive effects of quarantining information on differential risks need to be justified, not merely by fears of net negative public health effects, but by convincing evidence that such measures are actually warranted, that public health overall is in imminent danger and that the danger is sufficient to override principles of individual autonomy. Omitting such health-relevant information for consumers of such products effectively blindfolds them and impairs their making informed personal choices. Moral psychological issues that treat all tobacco/nicotine products similarly may also be influencing the reluctance to inform on differential risks. In countries where tobacco/nicotine products are legally sold and also differ greatly in disease risks compared to cigarettes (e.g., smokeless tobacco and vape), science-based, comprehensible, and actionable health information (consistent with health literacy principles) on differential risks should be available and only reconsidered if it is established that this information is causing losses to population health overall. Description of a method to support public health information management: organizational network analysis

In this case study we describe a method that has potential to provide systematic support for public health information management. Public health agencies depend on specialized information that travels throughout an organization via communication networks among employees. Interactions that occur within these networks are poorly understood and are generally unmanaged. We applied organizational network analysis, a method for studying communication networks, to assess the method’s utility to support decision making for public health managers, and to determine what links existed between information use and agency processes. Data on communication links among a health department’s staff was obtained via survey with a 93% response rate, and analyzed using Organizational Risk Analyzer (ORA) software. The findings described the structure of information flow in the department’s communication networks. The analysis succeeded in providing insights into organizational processes which informed public health managers’ strategies to address problems and to take advantage of network strengths. A power information user (PIU) model to promote information integration in Tennessee's public health community.

Observation and immersion in the user community are critical factors in designing and implementing informatics solutions; such practices ensure relevant interventions and promote user acceptance. Libraries can adapt these strategies to developing instruction and outreach. While needs assessment is typically a core facet of library instruction, sustained, iterative assessment underlying the development of user-centered instruction is key to integrating resource use into the workflow. This paper describes the Eskind Biomedical Library's (EBL's) recent work with the Tennessee public health community to articulate a training model centered around developing power information users (PIUs). PIUs are community-based individuals with an advanced understanding of information seeking and resource use and are committed to championing information integration. As model development was informed by observation of PIU workflow and information needs, it also allowed for informal testing of the applicability of assessment via domain immersion in library outreach. Though the number of PIUs involved in the project was small, evaluation indicated that the model was useful for promoting information use in PIU workgroups and that the concept of domain immersion was relevant to library-related projects. Moreover, EBL continues to employ principles of domain understanding inherent in the PIU model to develop further interventions for the public health community and library users.

To steer communication efforts in the midst of increasing access to personal genomic information, a self-regulation framework is presented. The framework emphasizes the importance of presenting a coherent message in all communiqués about public health genomics. Coherence should be based on an evidence-based model of how the public processes information about health conditions and an emphasis on risk-to-action links. Recommendations from the President's Council of Advisors for Science and Technology are reviewed as a way of identifying targets of opportunity for structured communications both within the healthcare system and in the broader external ecosystem of publicly available health information technologies. Building Data and Information Capacity in Environmental Public Health: A Best-Worst Scaling Experiment.

Environmental public health practitioners rely on information technology (IT) to maintain and improve environmental health. However, current systems have limited capacity. A better understanding of the importance of IT features is needed to enhance data and information capacity. (1) Rank IT features according to the percentage of respondents who rated them as essential to an information management system and (2) quantify the relative importance of a subset of these features using best-worst scaling. Information technology features were initially identified from a previously published systematic review of software evaluation criteria and a list of software options from a private corporation specializing in inspection software. Duplicates and features unrelated to environmental public health were removed. The condensed list was refined by a working group of environmental public health management to a final list of 57 IT features. The essentialness of features was electronically rated by environmental public health managers. Features where 50% to 80% of respondents rated them as essential (n = 26) were subsequently evaluated using best-worst scaling. Ontario, Canada. The majority of IT features (47/57) were considered essential to an information management system by at least half of the respondents (n = 52). The highest-rated features were delivery to printer, software encryption capability, and software maintenance services. Of the 26 features evaluated in the best-worst scaling exercise, the most important features were orientation to all practice areas, off-line capability, and ability to view past inspection reports and results. The development of a single, unified environmental public health information management system that fulfills the reporting and functionality needs of system users is recommended. A survey of suppression of public health information by Australian governments.

It is cause for concern when a democratically elected government suppresses embarrassing information by hindering public health research or the publication of research findings. We conducted a survey of Australian public health academics to estimate the level of acts of suppression of research by Australian governments, to characterise these events, and to gather views on what interventions might be effective in curbing them. A total of 302 academics in 17 institutions completed a postal questionnaire in August 2006 (46% of 652 invited). The instrument sought details of suppression events they had witnessed since 2001. There were 142 suppression events, including 85 personally experienced by 21.2% (n=64) of respondents. The rates were higher in 2005/06 than in earlier years. No State or Territory was immune from suppression. Although governments most commonly hindered research by sanitising, delaying or prohibiting publications (66% of events), no part of the research process was unaffected. Researchers commonly believed their work was targeted because it drew attention to failings in health services (48%), the health status of a vulnerable group (26%), or pointed to a harm in the environment (11%). The government agency seeking to suppress the health information mostly succeeded (87%) and, consequently, the public was left uninformed or given a false impression. Respondents identified a full range of participative, cognitive, structural and legislative control strategies. The suppression of public health information is widely practised by Australian governments. Systemic interventions are necessary to preserve the integrity of public health research conducted with government involvement.

The purpose of this review is to establish a conceptual framework on the role of information systems in public health care. Information is indispensable for effective management and development of health services and therefore considered as an important operational asset or resource. A Health Information System is mainly required to support management and operations at four levels: namely transactional and functional; operational control; management planning and control; and strategic planning. To provide the necessary information needs of users at these levels of management in the health care system, a structured information system coupled with appropriate information technology is required. Adequate and relevant information is needed regarding population characteristics, resources available and expended, output and outcome of health care activities. Additionally information needs to be reliable, accurate, timely, easily accessible and presented in a compact and meaningful form. With a well-planned health information system health authorities would be in a position to provide a quality, cost-effective and efficient health service for as many people as need it, optimal utilisation of resources and to maintain and improve the community's health status.

A qualitative study of health information technology in the Canadian public health system.

Although the adoption of health information technology (HIT) has advanced in Canada over the past decade, considerable challenges remain in supporting the development, broad adoption, and effective use of HIT in the public health system. Policy makers and practitioners have long recognized that improvements in HIT infrastructure are necessary to support effective and efficient public health practice. The objective of this study was to identify aspects of health information technology (HIT) policy related to public health in Canada that have succeeded, to identify remaining challenges, and to suggest future directions to improve the adoption and use of HIT in the public health system. A qualitative case study was performed with 24 key stakeholders representing national and provincial organizations responsible for establishing policy and strategic direction for health information technology. Identified benefits of HIT in public health included improved communication among jurisdictions, increased awareness of the need for interoperable systems, and improvement in data standardization. Identified barriers included a lack of national vision and leadership, insufficient investment, and poor conceptualization of the priority areas for implementing HIT in public health. The application of HIT in public health should focus on automating core processes and identifying innovative applications of HIT to advance public health outcomes. The Public Health Agency of Canada should develop the expertise to lead public health HIT policy and should establish a mechanism for coordinating public health stakeholder input on HIT policy.

Community health information exchanges have the characteristics of a public good, and they support population health initiatives at the state and national levels. However, current policy equally incentivizes health systems to create their own information exchanges covering more narrowly defined populations. Noninteroperable electronic health records and vendors' expensive custom interfaces are hindering health information exchanges. Moreover, vendors are imposing the costs of interoperability on health systems and community health information exchanges. Health systems are creating networks of targeted physicians and facilities by funding connections to their own enterprise health information exchanges. These private networks may change referral patterns and foster more integration with outpatient providers. The United States has invested billions of dollars to encourage the adoption of and implement the information technologies necessary for health information exchange (HIE), enabling providers to efficiently and effectively share patient information with other providers. Health care providers now have multiple options for obtaining and sharing patient information. Community HIEs facilitate information sharing for a broad group of providers within a region. Enterprise HIEs are operated by health systems and share information among affiliated hospitals and providers. We sought to identify why hospitals and health systems choose either to participate in community HIEs or to establish enterprise HIEs. We conducted semistructured interviews with 40 policymakers, community and enterprise HIE leaders, and health care executives from 19 different organizations. Our qualitative analysis used a general inductive and comparative approach to identify factors influencing participation in, and the success of, each approach to HIE.
