High cesarean section rates in Brazilian public hospitals and higher rates in private hospitals are well established. Less is known about the relationship between payment source and cesarean section rates within public and private hospitals. We analyzed the 1998, 2003, and 2008 rounds of a nationally representative household survey (PNAD), which includes type of delivery, where it took place, and who paid for it. We construct cesarean section rates for various categories, and perform logistic regression to determine the relative importance of independent variables on cesarean section rates for all births and first births only. Brazilian cesarean section rates were 42 percent in 1998 and 53 percent in 2008. Women who delivered publicly funded births in either public or private hospitals had lower cesarean section rates than those who delivered privately financed deliveries in public or private hospitals. Multivariate models suggest that older age, higher education, and living outside the Northeast region all positively affect the odds of delivering by cesarean section; effects are attenuated by the payment source-hospital type variable for all women and even more so among first births. Cesarean section rates have risen substantially in Brazil. It is important to distinguish payment source for the delivery to have a better understanding of those rates. © 2014 Wiley Periodicals, Inc.

The European Public Health Association (EUPHA) proposed and led PHIRE (Public Health Innovation and Research in Europe), with co-financing by the European Commission, to assess public health innovation and research at national level in Europe. PHIRE was also designed to promote organizational development and capacity building of EUPHA. We assess the success and limitations of using EUPHA's participative structures. In total, 30 European countries were included-27 EU countries, Iceland, Norway and Switzerland. EUPHA thematic section presidents were asked to identify country informants to report, through a web-based questionnaire, on eight public health innovations. National public health associations (EUPHA member organizations) were requested to identify their national public health research programmes and calls, review the health research system, coordinate a stakeholder workshop and provide a national report. The section and national reports were assessed for responses and completeness. Half of the final responding CIs were members of EUPHA sections and the other half gained from other sources. Experts declined to respond for reasons including lack of time, knowledge of the innovation or funding. National public health associations held PHIRE workshops with Ministries of Health in 14 countries; information for 10 countries was gained through discussions within the national association, or country visits by PHIRE partners. Six countries provided no response. Some national associations had too weak organizational structures for the work or insufficient financial resources or criticism of the project. EUPHA is the leading civil society organization giving support to public health research in Europe. PHIRE created new knowledge and supported organizational development. EUPHA sections gained expert reports on public health innovations in European countries and national public health associations reported on national public health research systems. Significant advances could be made if the European Commission worked more directly with EUPHA's expert members and with the national public health associations.

Caesarean birth rates in public and privately funded hospitals: a cross-sectional study.

To examine maternal and obstetric factors influencing births by cesarean section according to health care funding. A cross-sectional study with data from Southeastern Brazil. Caesarean section births from February 2011 to July 2012 were included. Data were obtained from interviews with women whose care was publicly or privately funded, and from their obstetric and neonatal records. Univariate and multivariate analyses were conducted to generate crude and adjusted odds ratios (OR) with 95% confidence intervals (95%CI) for caesarean section births. The overall caesarean section rate was 53% among 9,828 women for whom data were available, with the highest rates among women whose maternity care was privately funded. Reasons for performing a c-section were infrequently documented in women's maternity records. The variables that increased the likelihood of c-section regardless of health care funding were the following: paid employment, previous c-section, primiparity, antenatal and labor complications. Older maternal age, university education, and higher socioeconomic status were only associated with c-section in the public system. Higher maternal socioeconomic status was associated with greater likelihood of a caesarean section birth in publicly funded settings, but not in the private sector, where funding source alone determined the mode of birth rather than maternal or obstetric characteristics. Maternal socioeconomic status and private healthcare funding continue to drive high rates of caesarean section births in Brazil, with women who have a higher socioeconomic status more likely to have a caesarean section birth in all birth settings.

The American public`s perceptions regarding ionizing radiation do not always conform to or correlate with scientific evidence. The ultimate purpose of this coordinated Federal effort and report is to increase the public`s knowledge of the benefits and risks associated with ionizing radiation. This report is divided into five sections. The first section, Introduction, discusses the public`s knowledge of radiation, their perceptions of benefits versus risks, and the Federal government`s role in public education. The section also outlines the charge to the Subpanel. Radiation Issues and Public Reactions discusses several radiation issues important to Federal agencies for which public education programsmore » need to be established or enhanced. Federal Programs describes Federal agencies with public education programs on radiation and the nature of the programs they support. Education Issues and Federal Strategies explores the elements identified by the Subpanel as critical to the development and implementation of an effective Federal program in the area of public education on radiation issues and nuclear technologies. This paper presents an ongoing project targeting mobility support for users of public transportation including people with limited mobility. Existing approaches in this field mostly offer non-continuous guidance during the whole journey including multiple rides with different vehicles and footpaths in between at transfer points. Especially people with limited mobility, like people with disabilities and elderly people, or travelers who are not familiar with the specific route or transfer point, like tourists, often struggle with public transportation. They crave for a seamless approach covering all links of the mobility chain - the sequence of sections of the whole route - and providing comprehensive assistance throughout the whole journey. Previous projects and widespread experiences of project partners revealed that especially footpath sections are lacking proper support. In particular, the consortium identified three problem areas in existing approaches when dealing with footpath sections: (1) A lack of information, (2) a lack of orientation and (3) a lack of provision of services. In order to bridge (lat. PONS) these gaps in the mobility chain, new paradigms and technology concepts are developed to tackle the shortcomings on footpaths and combined in a toolkit to help developers of applications with focus on pedestrian navigation and public transport to improve their solutions with sustainable and state-of-the-art approaches.
