    Infection with the helminth Schistosoma (S.) mansoni drives the development of interleukin (IL)-10-producing regulatory B (Breg) cells in mice and man, which have the capacity to reduce experimental allergic airway inflammation and are thus of high therapeutic interest. However, both the involved antigen and cellular mechanisms that drive Breg cell development remain to be elucidated. Therefore, we investigated whether S. mansoni soluble egg antigens (SEA) directly interact with B cells to enhance their regulatory potential, or act indirectly on B cells via SEA-modulated macrophage subsets. Intraperitoneal injections of S. mansoni eggs or SEA significantly upregulated IL-10 and CD86 expression by marginal zone B cells. Both B cells as well as macrophages of the splenic marginal zone efficiently bound SEA in vivo, but macrophages were dispensable for Breg cell induction as shown by macrophage depletion with clodronate liposomes. SEA was internalized into acidic cell compartments of B cells and induced a 3-fold increase of IL-10, which was dependent on endosomal acidification and was further enhanced by CD40 ligation. IPSE/alpha-1, one of the major antigens in SEA, was also capable of inducing IL-10 in naïve B cells, which was reproduced by tobacco plant-derived recombinant IPSE. Other major schistosomal antigens, omega-1 and kappa-5, had no effect. SEA depleted of IPSE/alpha-1 was still able to induce Breg cells indicating that SEA contains more Breg cell-inducing components. Importantly, SEA- and IPSE-induced Breg cells triggered regulatory T cell development in vitro. SEA and recombinant IPSE/alpha-1 also induced IL-10 production in human CD1d+ B cells. In conclusion, the mechanism of S. mansoni-induced Breg cell development involves a direct targeting of B cells by SEA components such as the secretory glycoprotein IPSE/alpha-1. PMID:28753651

  3. Processes of Quantum Associative Memory (QuAM) Through New Maximally Entangled States (Singh-Rajput MES)

    Using Singh-Rajput MES as memory states in the evolutionary process of pattern storage and the non-evolutionary process of pattern recall (the two fundamental constituents of QuAM), the suitability and superiority of these MES over Bell's MES have been demonstrated in both these processes. It has been shown that, under the operations of all the possible memorization operators for a two-qubit system, the first two states of Singh-Rajput MES are useful for storing the pattern |11> and the last two of these MES are useful in storing the pattern |10> while Bell's MES are not much suitable as memory states in a valid memorization process. The recall operations have also been conducted by separately choosing Singh-Rajput MES and Bell's MES as memory states for possible various queries and it has been shown that in each case the choices of Singh-Rajput MES as valid memory states are much more suitable than those of Bell's MES.

  4. Bringing dust to good use: Quartz OSL ante-quam dating of the Strassberg rock avalanche (Northern Calcareous Alps, Austria) and implications for chronostratigraphic resolution of post-glacial deposits

    The Mieming massif in the western part of the Northern Calcareous Alps (NCA, Austria) records a complex history of rapid landscape change during the deglacial to paraglacial phase (c. 19.5-17 ka) after the Last Glacial Maximum (LGM). In this succession of changes, a major event that shaped the entire catchment till today was the descend of a rock avalanche of a GIS-estimated volume of 11 Mm3. This rock avalanche: (a) clogged a pre-existing valley, (b) dammed up an intramontane basin (Strassberg basin), and (c) triggered the incision of an epigenetic bedrock gorge some 1.5 km in length (Sanders et al., 2016). Geomorphological and sedimentological indicators all suggest that the rock avalanche descended very soon after local deglaciation, but an age estimate of mass-wasting was difficult to provide. Bulk radiocarbon ages of the acid-washed, humic fraction of soil horizons intercalated into colluvium above the rock avalanche deposit indicated an oldest age of 11180-11170 a cal BP; a large scatter of radiocarbon ages (youngest: 7960 a cal BP; oldest: 11180 a cal BP; total of three ages) indicated that these well-drained soils were subject to input of younger humic substance, thus can provide only a crude proxy ante-quam date for the event. Over the past two years, in the NCA, a landscape-wide drape of polymictic siliciclastic aeolian silt was discovered that - as suggested by its geomorphic and sedimentary context - most probably was deposited during the late-glacial chron. The drape is verified over a vertical relief amplitude of more than 2000 meters, from valley floors up to LGM nunataks (Gild et al., 2016). A level of polymictic siliciclastic silt was found also directly on top of the Strassberg rock avalanche deposit. This provided an opportunity to deduce a more precise ante-quam quartz OSL age of 18.77 ± 1.55 ka for mass-wasting.   5. Are some people less influenced by others' opinions? The role of internal political self-efficacy and need for cognition in impression formation on social networking sites.

    While social information processing theory (SIP) asserts that Internet users form impressions of others by using available cues in the web environment, questions remain regarding individual differences in dependence on such cues. Focusing on others' opinion cues, this study investigates the role of internal political self-efficacy (IPSE) and need for cognition (NFC) as potential moderators of the cue effect. Participants (N=251) were randomly exposed to a fictitious political candidate's Facebook profile displaying either positive or negative comments and then asked to rate perceived trustworthiness and expertise of the candidate. The results indicated that although others' opinions were powerful cues, their influence was moderated by IPSE; individuals with higher IPSE were less influenced by others' opinions. The two-way interaction was significantly moderated by NFC such that those with low IPSE and low NFC were most susceptible to others' influences, thereby indicating a significant three-way interaction among IPSE, NFC, and others' opinion cues.

    Interprofessional education is intended to train practitioners to collaboratively address challenges in healthcare delivery, and interprofessional simulation-based education (IPSE) provides realistic, contextual learning experiences in which roles, responsibilities, and professional identity can be learned, developed, and assessed. Reducing negative stereotypes within interprofessional relationships is a prime target for IPSE. We sought to understand whether perceptions of interprofessional education and provider stereotypes change among nursing and medical students after participating in IPSE. We also sought to determine whether changes differed based on the student's discipline. This was a quasi-experimental pretest-posttest study. The study took place at a large mid-Atlantic public university with a comprehensive health science campus. 147 senior Bachelors of Science in Nursing students and 163 fourth-year medical students participated. Students were grouped into interprofessional teams for a two-week period and participated in three two-hour simulations focused on collaboration around acutely ill patients. At the beginning of the first session, they completed a pretest survey with demographic items and measures of their perceptions of interprofessional clinical education, stereotypes about doctors, and stereotypes about nurses. They completed a posttest with the same measures after the third session. 251 students completed both the pretest and posttest surveys. On all three measures, students showed an overall increase in scores after the IPSE experience. In comparing the change by student discipline, medical students showed little change from pretest to posttest on stereotypes of doctors, while nursing students had a significant increase in positive perceptions about doctors.     The harmonisation of training programmes for infection control and hospital hygiene (IC/HH) professionals in Europe is a requirement of the Council recommendation on patient safety. The European Centre for Disease Prevention and Control commissioned the 'Training Infection Control in Europe' project to develop a consensus on core competencies for IC/HH professionals in the European Union (EU). Core competencies were drafted on the basis of the Improving Patient Safety in Europe (IPSE) project's core curriculum (CC), evaluated by questionnaire and approved by National Representatives (NRs) for IC/HH training. NRs also re-assessed the status of IC/HH training in European countries in 2010 in comparison with the situation before the IPSE CC in 2006. The IPSE CC had been used to develop or update 28 of 51 IC/HH courses. Only 10 of 33 countries offered training and qualification for IC/HH doctors and nurses. The proposed core competencies are structured in four areas and 16 professional tasks at junior and senior level. They form a reference for standardisation of IC/HH professional competencies and support recognition of training initiatives.

    This study explores how interprofessional simulation-based education (IPSE) can contribute to a change in students' understanding of teamwork and professional roles. A series of 1-day training sessions was arranged involving undergraduate nursing and medical students. Scenarios were designed for practicing teamwork principles and interprofessional communication skills by endorsing active participation by all team members. Four focus groups occurred 2-4 weeks after the training. Thematic analysis of the transcribed focus groups was applied, guided by questions on what changes in students' understanding of teamwork and professional roles were identified and how such changes had been achieved. The first question, aiming to identify changes in students' understanding of teamwork, resulted in three categories: realizing and embracing teamwork fundamentals, reconsidering professional roles, and achieving increased confidence. The second question, regarding how participation in IPSE could support the transformation of students' understanding of teamwork and of professional roles, embraced another three categories: feeling confident in the learning environment, embodying experiences, and obtaining an outside perspective. This study showed the potential of IPSE to transform students' understanding of others' professional roles and responsibilities. Students displayed extensive knowledge on fundamental teamwork principles and what these meant in the midst of participating in the scenarios. A critical prerequisite for the development of these new insights was to feel confident in the learning environment. The significance of how the environment was set up calls for further research on the design of IPSE in influencing role understanding and communicative skills in significant ways.

  12. Contrast ratio and masking ability of three ceramic veneering materials.

    Porcelain veneer materials are translucent and are therefore affected by their thickness as well as the color of the underlying substructure, which limits their masking ability and compromises the esthetic result in heavily stained teeth. The purpose of this study was to compare the contrast ratio (CR) and masking ability of three different veneering ceramics with two thicknesses by measuring the color differences over white and black backgrounds. Correlations between CR and masking ability of these veneering ceramics were evaluated. A total of 30 disc-shaped specimens (12 mm diameter × 1.0 mm or 1.5 mm) were fabricated in shade A2 from three types of all-ceramic systems: IPS e.max Press (IPSe; Ivoclar Vivadent, Schaan, Liechtensein), Vita VM7 (VM7; VITA Zahnfabrik, Bad Säckingen, Germany), and Nobel Rondo Press Alumina: Solo (NRPA; Nobel Biocare, Zürich-Flughafen, Switzerland). The CR, defined as the ratio of illuminance (Y) of the test material when placed on the black background (Yb) to the illuminance of the same material when placed over a white background (Yw), was determined (CR=Yb/Yw). The color (CIE L*a*b*) and Y of each specimen were measured over standard white and black tiles using a spectrophotometer (ColorEye 7000 A, Model C6, GretagMacbeth, New Windsor, NY, USA). Masking abilities of the specimens were determined by measuring the color difference (ΔE) over white and black backgrounds. Both CR and ΔE data were analyzed using two-way analysis of variance (ANOVA). One-way ANOVA was used to compare the mean values of CR across the three materials followed by the Duncan multiple comparison test. The correlations between CR and ΔE were determined by comparing R(2) values obtained from a linear regression analysis. A Student t-test for independent samples was used to compare the mean contrast ratio and ΔE values for the two thicknesses. CR values of NRPA were significantly less than those of IPSe and VM7, and the CR of IPSe was higher than that of VM

    New set of maximally entangled states (Singh-Rajput MES), constituting orthonormal eigen bases, has been revisited and its superiority and suitability in pattern-association (Quantum Associative Memory, QuAM) have been demonstrated. Using these MES as memory states in the evolutionary process of pattern storage in a two-qubit system, it has been shown that the first two states of Singh-Rajput MES are useful for storing the pattern |11> and the last two of these MES are useful in storing the pattern |10> Recall operations of quantum associate memory (QuAM) have been conducted through evolutionary process in terms of unitary operators by separately choosing Singh-Rajput MES and Bell's MES as memory states and it has been shown that Singh-Rajput MES as valid memory states for recalling the patterns in a two-qubit system are much more suitable than Bell's MES.

    The NSF-funded Internships in Public Science Education (IPSE) program provides a unique opportunity for undergraduate and graduate students with varied academic background to experience learning and teaching science--specifically nanotechnology--to the general public and middle-school students. The program is in collaboration with Discovery World Museum of Milwaukee, Wisconsin. IPSE interns have created a number of classroom activities ranging from understanding the scale of a nanometer to experimenting with liquid crystal sensors to critically examining the societal implications of nanotechnology. In a new phase of the program, the interns are developing a museum exhibit on nanotechnology to be housed at the Discovery World Museum. Through this experience, intern teams learn about nanotechnology, brainstorm ideas, present and receive feedback on their ideas, and create an exhibit prototype to explain nanotechnology and related science concepts. The program also focuses on professional development, during which interns learn techniques for presenting to non-technical audiences, strategies for assessing their materials, and work on their skills in teamwork, project design, leadership, and science communication.

    The objective of this study was to investigate the interaction of infrared laser light on Computer Aided Design and Computer Aided Manufacturing (CAD/CAM) ceramic surfaces. Sixty CAD/CAM ceramic discs were prepared and divided into two different groups: lithiumdisilicate ceramic (IPSe.maxCADs) and Zirconia ceramic (IPSe.maxZirCADs). The laser irradiation was performed on graphite and non-graphite surfaces with a Carbon Dioxide laser at 5W and 10W power in continuous mode (CW mode) and with Neodymium Yttrium Aluminum Perovskite (Nd:YAP) laser at 10W. Surface textures and compositions were examined using Scanning Electron Microscopy (SEM), and Energy Dispersive Spectroscopy (EDS). Thermal elevation was measured by thermocouple during laser irradiation. The SEM observation showed a rough surface plus cracks and fissures on CO2 10W samples and melting areas in Nd:YAP samples; moreover, with CO2 5W smooth and shallow surfaces were observed. EDS analysis revealed that laser irradiation does not result in modifications of the chemical composition even if minor changes in the atomic mass percentage of the components were registered. Thermocouple showed several thermal changes during laser irradiation. CO2 and Nd:YAP lasers modify CAD/CAM ceramic surface without chemical composition modifications.

  2. Antigenic cross-reactivity between Schistosoma mansoni and peanut: a role for cross-reactive carbohydrate determinants (CCDs) and implications for the hygiene hypothesis.

    The antigenic reactivity of constituents of Schistosoma mansoni and peanut (Arachis hypogaea) was investigated to determine whether identical antigenic epitopes possessed by both organisms provided a possible explanation for the negative correlation between chronic schistosome infection and atopy to allergens. Aqueous extracts of peanuts were probed in Western immunoblots with rabbit IgG antibodies raised against the egg, cercarial and adult worm stages of S. mansoni. Several molecules in the peanut extract were antigenically reactive with antibodies from the various rabbit anti-schistosome sera. A pair of cross-reactive peanut molecules at ~30 000-33 000 molecular weight was purified and both proteins were identified by mass spectrometric analysis as the peanut allergen Ara h 1. Anti-S. mansoni soluble egg antigen antibodies that were eluted off the peanut molecules reacted with two S. mansoni egg antigens identified by mass spectrometry as IPSE/α-1 and κ-5. Alignments of the amino acid sequences of Ara h 1 and either IPSE/α-1 or κ-5 revealed a low level of peptide sequence identity. Incubation of nitrocellulose paper carrying electrophoresed peanut molecules, six constituents of other allergic plants and S. mansoni egg antigens in a mild solution of sodium metaperiodate before probing with antibodies, inhibited most of the cross-reactivities. The results are consistent with the antigenic cross-reactive epitopes of S. mansoni egg antigens, peanut and other allergic plants being cross-reactive carbohydrate determinants (CCDs). These findings are novel and an explanation based on 'blocking antibodies' could provide an insight for the inverse relationship observed between schistosome infection and allergies. © 2017 John Wiley & Sons Ltd.

    The knowledge of anthrax as a disease and its importance as a zoonosis in the Greco-Roman world is revealed through a selection of classical texts and mythological sources, taking into account evidence of reworking and reuse of these texts up until the nineteenth century. The numerous names given to the disease throughout history and their linguistic origins will also be examined in this paper. The narrative of the epizoonoses in Noricum in Virgil's Georgics; taken by several to represent a description of an anthrax epidemic, and which had a great influence in written works on veterinary medicine up until the discovery of bacteria, will be given particular attention. The crucial term is "Sacer Ignis", synonymous for several different human and animal diseases through time. This term will be analysed in terms of linguistic origin and the changes in meaning it acquired throughout the centuries.

    Dozens of scientific textbooks [e.g., Spaldin, 2011, p. v; Krijgsman and Langereis, 2009, p. 252; Prölls, 2004, p. 211; Merrill et al., 1996, p. 7; Livingston, 1996, p. 27; Blakely, 1996, pp. xiv, 154; Gillmor, 1990, p. 9] attribute the famous dictum magnus magnes ipse est globus terrestris ("the terrestrial globe is itself a big magnet") to the English physician and scientist William Gilbert (1544-1603). It is repeatedly claimed that these words were contained in the title of Gilbert's book or one of his chapters [e.g., Carlowicz and Lopez, 2002, n.p.; Courtillot, 2002, pp. 26, 49; Lang and Whitney, 1991, p. 120]. Certainly, they convey the thrust of Gilbert's De Magnete, in which it was argued for the first time that the Earth sustains its own magnetic dipole field, on the basis of experimentation on magnets.

  5. How Native Prosody Affects Pitch Processing during Word Learning in Limburgian and Dutch Toddlers and Adults

    In this study, Limburgian and Dutch 2.5- to 4-year-olds and adults took part in a word learning experiment. Following the procedure employed by Quam and Swingley (2010) and Singh et al. (2014), participants learned two novel word-object mappings. After training, word recognition was tested in correct pronunciation (CP) trials and mispronunciation (MP) trials featuring a pitch change. Since Limburgian is considered a restricted tone language, we expected that the pitch change would hinder word recognition in Limburgian, but not in non-tonal Dutch listeners. Contrary to our expectations, both Limburgian and Dutch children appeared to be sensitive to pitch changes in newly learned words, indicated by a significant decrease in target fixation in MP trials compared to CP trials. Limburgian and Dutch adults showed very strong naming effects in both trial types. The results are discussed against the background of the influence of the native prosodic system. PMID:29018382

    Recall operations of quantum associative memory (QuAM) have been conducted separately through evolutionary as well as non-evolutionary processes in terms of unitary and non- unitary operators respectively by separately choosing our recently derived maximally entangled states (Singh-Rajput MES) and Bell's MES as memory states for various queries and it has been shown that in each case the choices of Singh-Rajput MES as valid memory states are much more suitable than those of Bell's MES. it has been demonstrated that in both the types of recall processes the first and the fourth states of Singh-Rajput MES are most suitable choices as memory states for the queries `11' and `00' respectively while none of the Bell's MES is a suitable choice as valid memory state in these recall processes. It has been demonstrated that all the four states of Singh-Rajput MES are suitable choice as valid memory states for the queries `1?', `?1', `?0' and `0?' while none of the Bell's MES is suitable choice as the valid memory state for these queries also.

  7. Zoonotic helminths affecting the human eye

    Nowaday, zoonoses are an important cause of human parasitic diseases worldwide and a major threat to the socio-economic development, mainly in developing countries. Importantly, zoonotic helminths that affect human eyes (HIE) may cause blindness with severe socio-economic consequences to human communities. These infections include nematodes, cestodes and trematodes, which may be transmitted by vectors (dirofilariasis, onchocerciasis, thelaziasis), food consumption (sparganosis, trichinellosis) and those acquired indirectly from the environment (ascariasis, echinococcosis, fascioliasis). Adult and/or larval stages of HIE may localize into human ocular tissues externally (i.e., lachrymal glands, eyelids, conjunctival sacs) or into the ocular globe (i.e., intravitreous retina, anterior and or posterior chamber) causing symptoms due to the parasitic localization in the eyes or to the immune reaction they elicit in the host. Unfortunately, data on HIE are scant and mostly limited to case reports from different countries. The biology and epidemiology of the most frequently reported HIE are discussed as well as clinical description of the diseases, diagnostic considerations and video clips on their presentation and surgical treatment.   8. Cross-Reactivity between Schistosoma mansoni Antigens and the Latex Allergen Hev b 7: Putative Implication of Cross-Reactive Carbohydrate Determinants (CCDs)

    IgG antibodies produced by rabbits immunized against S. mansoni antigens cross-reacted with aqueous soluble constituents of a variety of allergens. The antibody cross-reactivity was largely sensitive to degradation by treatment of the target antigens with sodium meta-periodate, suggesting the cross-reactivity was due to carbohydrate determinants that were common to both the schistosome and the allergens (CCDs). The reaction between the rabbit antibodies and a 43 kDa molecule in a rubber latex extract was analysed further: tandem mass spectrometry identified the latex molecule as allergen Hev b 7. Rabbit anti-schistosome IgG antibodies purified by acid-elution from solid-phase latex Hev b 7 reacted with the S. mansoni egg antigens IPSE/alpha-1 and kappa-5 and cercarial antigens SPO-1 and a fatty acid-binding protein. Moreover, purified anti-S. mansoni egg, latex cross-reactive antibodies reacted with antigenic constituents of some fruits, a result of potential relevance to the latex-fruit syndrome of allergic reactions. We propose that IgG anti-schistosome antibodies that cross-react with allergens may be able to block IgE-induced allergic reactions and thus provide a possible explanation for the hygiene hypothesis.   9. Becoming a nurse: "it's just who I am".

    In any research study, researchers situate themselves, either explicitly or implicitly, within a variety of frameworks when studying phenomena. From a research perspective, the study will be more robust if these frameworks and the accompanying assumptions are compatible with each other; otherwise, the project may lack coherence. Ricoeur offers a methodological perspective-that is, an interpretive theory as reflected in mimesis, which is congruent with his ontological theory of self identity (ipse- and idem-identity). To illustrate Ricoeur's frameworks when researching the self identities, I use examples from a research study in which I asked senior nursing students to explore their experience of becoming a nurse. I do not intend for this article to be a comprehensive research report, but I present it as an exemplar of how Ricoeur's ideas can guide other researchers studying self identity. I labelled my study a narrative research project and assumed that becoming a nurse means developing a self identity as a nurse. While self identity is often framed in psychological terms, Ricoeur uses a philosophical perspective when exploring this concept. I conclude the paper by suggesting (a) that Ricoeur can guide any project in which researchers ask participants to describe "becoming" a person with illness, sickness or disease, and (b) that educators of healthcare professional students can improve the educative experience by purposefully considering how a student's ontological self affects that student's practice.

    Background The field of dental ceramics for CAD-CAM is enriched with a new innovative material composition having a porous three-dimensional structure of feldspathic ceramic infiltrated with acrylic resins.The aim of this study is to determine the mechanical properties of Polymer-Infiltrated-Ceramic-Network (PICN) and compare its performance with other ceramics and a nano-ceramic resin available for CAD-CAM systems. Material and Methods In this study a total of five different materials for CAD-CAM were investigated. A polymer-infiltrated ceramic (Vita Enamic), a nano-ceramic resin (Lava Ultimate), a feldspathic ceramic (Mark II), a lithium disilicate ceramic (IPS-e max CAD) and finally a Leucite based ceramic (Empress - CAD). From CAD-CAM blocks, 120 bars (30 for each material cited above) were cut to measure the flexural strength with a three-point-bending test. Strain at failure, fracture stress and Weibull modulus was calculated. Vickers hardness of each material was also measured. Results IPS-EMAX presents mechanical properties significantly better from the other materials studied. Its strain at failure, flexural strength and hardness exhibited significantly higher values in comparison with the others. VITA ENAMIC and LAVA ULTIMATE stand out as the next most resistant materials. Conclusions The flexural strength, elastic modulus similar to a tooth as well as having less hardness than ceramics make PICN materials an option to consider as a restorative material.   12. Mater semper incertus est: who's your mummy?

    In English law, the legal term for father has been given a broad definition but the definition of mother remains rooted in biology with the Roman law principle mater semper certa est (the mother is always certain) remaining the norm. However, motherhood may be acquired through giving birth to a child, by donation of gametes or by caring and nurturing a child so that the identity of the mother is no longer certain particularly in the case of surrogacy arrangements. While the law in the UK may automatically recognise the parental status of a commissioning father in a traditional surrogacy arrangement, the parental status of the commissioning mother is not automatically recognised in either a traditional or a gestational surrogacy arrangement. Thus the maxim mater est quam gestation demonstrat (meaning the mother is demonstrated by gestation) is also not approached consistently in the legal interpretation of parentage or motherhood in surrogacy as against other assisted reproduction methods. This raises questions about the extent to which motherhood should be affected by the method of reproduction and whether the sociological and philosophical concept of motherhood should, in the case of surrogacy, give rise to a new principle of 'mater semper incertus est' (the mother is uncertain). This article will argue that the time has come to move away from a legal definition of 'mother' that is based on biology to one that recognises the different forms of motherhood. © The Author [2014].   13. Quality assurance of medicines supplied to low-income and middle-income countries: poor products in shiny boxes?

    Objective In today's context of globalisation of pharmaceutical production and distribution, international and national procurement agencies play a de facto key role in defining the quality of medicines available in sub-Saharan Africa. We evaluated the compliance of a sample of pharmaceutical distributors active in sub-Saharan Africa with the standards of the WHO guideline ‘Model Quality Assurance System (WHO MQAS) for procurement agencies’, and we investigated factors favouring or hindering the adequate implementation of the guideline. Methods We used mixed-methods methodology to analyse quantitative and qualitative data. The quantitative study consisted of a retrospective secondary analysis of data collected by QUAMED (Quality Medicines for all), a partnership that pleads for universal access to quality-assured medicines. The qualitative survey consisted of formal and informal interviews with key informants. We adopted an embedded multiple-case study design. Findings Our analysis suggests that international distributors based in Europe perform, on average, better than sub-Saharan African distributors. However, some weaknesses are ubiquitous and concern critical processes, such as the initial selection of the products and the ongoing reassessment of their quality. This is due to several different factors: weak regulatory oversight, insufficient human/financial resources, weak negotiating power, limited judicial autonomy and/or lack of institutional commitment to quality. Conclusions Our findings suggest that pharmaceutical distributors active in sub-Saharan Africa generally do not apply stringent criteria for selecting products and suppliers. Therefore, product quality is not consistently assured but depends on the requirements of purchasers. While long-term solutions are awaited, the WHO MQAS guideline should be used as an evaluation and training tool to upgrade current standards. PMID:28589013

We assessed the distributors' compliance with good distribution practices (GDP), general quality requirements (GQR) and cold chain management (CCM), based on an evaluation tool inspired by the WHO guidelines 'Model Quality Assurance System (MQAS) for procurement agencies'. Descriptive statistics describe the compliance for the whole sample, for distributors in sub-Saharan Africa (SSA) versus those in non-SSA, and for those in low-income countries (LICs) versus middle-income countries (MICs). Local private pharmaceutical distributors in our sample were non-compliant, very low-compliant or low-compliant for GQR (70%), GDP (60%) and CCM (41%). Only 7/60 showed good to full compliance for at least two criteria. Observed compliance varies by geographical region and by income group: maximum values are higher in non-SSA versus SSA and in MICs versus LICs, while minimum values are the same across different groups. The poor compliance with WHO quality standards observed in our sample indicates a concrete risk that patients in LMICs are exposed to poor-quality or degraded medicines. Significant investments are needed to strengthen the regulatory supervision, including on private pharmaceutical distributors. An adapted standardised evaluation tool inspired by the WHO MQAS would be helpful for self-evaluation, audit and inspection purposes.

  16. SST algorithms in ACSPO reanalysis of AVHRR GAC data from 2002-2013

    In response to a request from the NOAA Coral Reef Watch Program, NOAA SST Team initiated reprocessing of 4 km resolution GAC data from AVHRRs flown onboard NOAA and MetOp satellites. The objective is to create a longterm Level 2 Advanced Clear-Sky Processor for Oceans (ACSPO) SST product, consistent with NOAA operations. ACSPO-Reanalysis (RAN) is used as input in the NOAA geo-polar blended Level 4 SST and potentially other Level 4 SST products. In the first stage of reprocessing (reanalysis 1, or RAN1), data from NOAA-15, -16, -17, -18, -19, and Metop-A and -B, from 2002-present have been processed with ACSPO v2.20, and matched up with quality controlled in situ data from in situ Quality Monitor (iQuam) version 1. The ~12 years time series of matchups were used to develop and explore the SST retrieval algorithms, with emphasis on minimizing spatial biases in retrieved SSTs, close reproduction of the magnitudes of true SST variations, and maximizing temporal, spatial and inter-platform stability of retrieval metrics. Two types of SST algorithms were considered: conventional SST regressions, and recently developed incremental regressions. The conventional equations were adopted in the EUMETSAT OSI-SAF formulation, which, according to our previous analyses, provide relatively small regional biases and well-balanced combination of precision and sensitivity, in its class. Incremental regression equations were specifically elaborated to automatically correct for model minus observation biases, always present when RTM simulations are employed. Improved temporal stability was achieved by recalculation of SST coefficients from matchups on a daily basis, with a +/-45 day window around the current date. This presentation describes the candidate SST algorithms considered for the next round of ACSPO reanalysis, RAN2.

HDX is designed to provide a laboratory experimental platform to reproduce the earth's magnetospheric structure for investigations on the mechanism of acceleration/loss and wave-particle interaction of energetic particles in radiation belt, and on the influence of magnetic storms on the inner magnetosphere. It can be operated together with Harbin Reconnection eXperiment (HRX), which is another part of SPERF, to study the fundamental processes during interactions between solar wind and Earth's magnetosphere. In this presentation, the scientific goals and experimental plans for HDX, together with the means applied to generate the plasma with desired parameters, including multiple plasma sources and different kinds of coils with specific functions, as well as advanced diagnostics designed to be equipped to the facility for multi-functions, are reviewed.   18. Joint inversion of geophysical data using petrophysical clustering and facies deformation wth the level set technique

    Geological expertise and petrophysical relationships can be brought together to provide prior information while inverting multiple geophysical datasets. The merging of such information can result in more realistic solution in the distribution of the model parameters, reducing ipse facto the non-uniqueness of the inverse problem. We consider two level of heterogeneities: facies, described by facies boundaries and heteroegenities inside each facies determined by a correlogram. In this presentation, we pose the geophysical inverse problem in terms of Gaussian random fields with mean functions controlled by petrophysical relationships and covariance functions controlled by a prior geological cross-section, including the definition of spatial boundaries for the geological facies. The petrophysical relationship problem is formulated as a regression problem upon each facies. The inversion of the geophysical data is performed in a Bayesian framework. We demonstrate the usefulness of this strategy using a first synthetic case for which we perform a joint inversion of gravity and galvanometric resistivity data with the stations located at the ground surface. The joint inversion is used to recover the density and resistivity distributions of the subsurface. In a second step, we consider the possibility that the facies boundaries are deformable and their shapes are inverted as well. We use the level set approach to perform such deformation preserving prior topological properties of the facies throughout the inversion. With the help of prior facies petrophysical relationships and topological characteristic of each facies, we make posterior inference about multiple geophysical tomograms based on their corresponding geophysical data misfits. The method is applied to a second synthetic case showing that we can recover the heterogeneities inside the facies, the mean values for the petrophysical properties, and, to some extent, the facies boundaries using the 2D joint inversion of

    There are a growing number of level 4 (L4; gap-free gridded) sea surface temperature (SST) products generated by blending SST data from various sources which are available for use in a wide variety of operational and scientific applications. In most cases, each product has been developed for a specific user community with specific requirements guiding the design of the product. Consequently differences between products are implicit. In addition, anomalous atmospheric conditions, satellite operations and production anomalies may occur which can introduce additional differences. This paper describes a new web-based system called the L4 SST Quality Monitor (L4-SQUAM) developed to monitor the quality of L4 SST products. L4-SQUAM intercompares thirteen L4 products with 1-day latency in an operational environment serving the needs of both L4 SST product users and producers. Relative differences between products are computed and visualized using maps, histograms, time series plots and Hovmöller diagrams, for all combinations of products. In addition, products are compared to quality controlled in situ SST data (available from the in situ SST Quality Monitor, iQUAM, companion system) in a consistent manner. A full history of products statistics is retained in L4-SQUAM for time series analysis. L4-SQUAM complements the two other Group for High Resolution SST (GHRSST) tools, the GHRSST Multi Product Ensemble (GMPE) and the High Resolution Diagnostic Data Set (HRDDS) systems, documented in part 1 of this paper and elsewhere, respectively. Our results reveal significant differences between SST products in coastal and open ocean areas. Differences of >2 °C are often observed at high latitudes partly due to different treatment of the sea-ice transition zone. Thus when an ice flag is available, the intercomparisons are performed in two ways: including and excluding ice-flagged grid points.   20. Concordance between European and US case definitions of healthcare-associated infections

    Background Surveillance of healthcare-associated infections (HAI) is a valuable measure to decrease infection rates. Across Europe, inter-country comparisons of HAI rates seem limited because some countries use US definitions from the US Centers for Disease Control and Prevention (CDC/NHSN) while other countries use European definitions from the Hospitals in Europe Link for Infection Control through Surveillance (HELICS/IPSE) project. In this study, we analyzed the concordance between US and European definitions of HAI. Methods An international working group of experts from seven European countries was set up to identify differences between US and European definitions and then conduct surveillance using both sets of definitions during a three-month period (March 1st -May 31st, 2010). Concordance between case definitions was estimated with Cohen’s kappa statistic (κ). Results Differences in HAI definitions were found for bloodstream infection (BSI), pneumonia (PN), urinary tract infection (UTI) and the two key terms “intensive care unit (ICU)-acquired infection” and “mechanical ventilation”. Concordance was analyzed for these definitions and key terms with the exception of UTI. Surveillance was performed in 47 ICUs and 6,506 patients were assessed. One hundred and eighty PN and 123 BSI cases were identified. When all PN cases were considered, concordance for PN was κ = 0.99 [CI 95%: 0.98-1.00]. When PN cases were divided into subgroups, concordance was κ = 0.90 (CI 95%: 0.86-0.94) for clinically defined PN and κ = 0.72 (CI 95%: 0.63-0.82) for microbiologically defined PN. Concordance for BSI was κ = 0.73 [CI 95%: 0.66-0.80]. However, BSI cases secondary to another infection site (42% of all BSI cases) are excluded when using US definitions and concordance for BSI was κ = 1.00 when only primary BSI cases, i.e. Europe-defined BSI with ”catheter” or “unknown” origin and US-defined laboratory-confirmed BSI (LCBI), were

    The story of the quest to understand Earth’s magnetic field is one of the longest and richest in the history of science. It weaves together Greek philosophy, Chinese mysticism, the development of the compass and navigation, the physics of electromagnetism and the jig-saw like piecing together of the internal structure of the planet beneath our feet. The story begins with Magnes, an old shepherd, trudging up the mountainside after a violent thunder storm, astonished at how the iron studs in his boots stick to the rocks. It was Alexander von Humboldt who, three millennia on, pointed to lightning as the source of such magnetization. The first compass was made 2000 years ago in China - to divine the ways of feng shui - a guide to planting crops, planning streets, orienting buildings and more. It reached Europe as a navigational tool in the 12th century - no-one is quite sure how, but en route it changed from south-pointing to the north-pointing compasses of today. The earliest truly scientific experiments and writings concerned magnets and geomagnetism: Petrus Peregrinus’ Epistola of 1269, and William Gilbert’s De Magnete of1600, in which he declared Magnus magnes globus terrestris ipse est - the Earth itself is a great magnet. By then it was recognized that the compass didn’t point exactly north, and the discrepancy varied from place to place and changed over time - something of a problem for Gilbert’s idea of a geocentric axial dipole. However declination and secular variation were problems well known to Edmund Halley, who, in 1700, charted the angle of declination over the Atlantic Ocean, and in the process introduced the Halleyan line - the contour. Many of the world’s greatest scientists have turned their minds to the problem of magnetism and geomagnetism in particular - Coulomb, Gauss, Faraday, Maxwell - yet in 1905, Einstein described geomagnetism as “one of the great unsolved problems of physics”.
