Request forms are important means of communication between physicians and diagnostic service providers. Pre-analytical errors account for over two thirds of errors encountered in diagnostic service provision. The importance of adequate completion of request forms is usually underestimated by physicians which may result in medical errors or delay in instituting appropriate treatment. The aim of this study was to audit the level of completion of request forms presented at a multidisciplinary diagnostic center. A review of all requests forms for investigations which included radiologic, laboratory and cardiac investigations received between July and December 2011 was performed to assess their level of completeness. The data was entered into a spreadsheet and analyzed. Only 1.3% of the 7,841 request forms reviewed were fully completed. Patient's names, the referring physician's name and gender were the most completed information on the forms evaluated with 99.0%, 99.0% and 90.3% completion respectively. Patient's age was provided in 68.0%, request date in 88.2%, and clinical notes/ diagnosis in 65.9% of the requests. Patient's full address was provided in only 5.6% of requests evaluated. This study shows that investigation request forms are inadequately filled by physicians in our environment. Continuous medical education of physicians on the need for adequate completion of request forms is needed.

Imported infections: What information should be collected by surveillance systems to inform public health policy?

International travel carries the risk of imported diseases, which are an increasingly significant public health problem. There is little guidance about which variables should be collected by surveillance systems for strategy-based surveillance. Surveillance forms for dengue, malaria, hepatitis A, typhoid and measles were collected from Australia and New Zealand and information on these compared with national surveillance forms from the UK and Canada by travel health experts. Variables were categorised by information relating to recent travel, demographics and disease severity. Travel-related information most commonly requested included country of travel, vaccination status and travel dates. In Australia, ethnicity information requested related to indigenous status, whilst in New Zealand it could be linked to census categories. Severity of disease information most frequently collected were hospitalisation and death. Reviewing the usefulness of variables collected resulted in the recommendation that those included should be: overseas travel, reason for travel, entry and departure dates during the incubation period, vaccination details, traveller's and/or parents' country of birth, country of usual residence, time resident in current country, postcode, hospitalisation and death details. There was no agreement about whether ethnicity details should be collected. The inclusion of these variables on surveillance forms could enable imported infection-related policy to be formulated nationally and internationally. Copyright © 2016 Elsevier Ltd. This article illustrates how Scottish health professionals involved in contemporary abortion provision construct stratified expectations about women’s reproductive decision-making. Drawing on 42 semi-structured interviews I reveal the contingent discourses through which health professionals constitute the ‘rationality’ of the female subject who requests abortion. Specifically, I illustrate how youth, age, parity and class are mobilised as criteria through which to distinguish ‘types’ of patient whose requests for abortion are deemed particularly understandable or particularly problematic. I conceptualise this process of differentiation as a form of ‘stratified reproduction’ (Colen, 1995; Ginsburg and Rapp, 1995) and argue that it is significant for two reasons. Firstly, it illustrates the operation of dominant discourses concerning abortion and motherhood in twenty-first century Britain. Secondly, it extends the forms of critique which feminist scholarship has, to date, developed of the regulation of abortion provision in the UK. PMID:25774067

During social interaction, actions, and words may be expressed in different ways, for example, gently or rudely. A handshake can be gentle or vigorous and, similarly, tone of voice can be pleasant or rude. These aspects of social communication have been named vitality forms by Daniel Stern. Vitality forms represent how an action is performed and characterize all human interactions. In spite of their importance in social life, to date it is not clear whether the vitality forms expressed by the agent can influence the execution of a subsequent action performed by the receiver. To shed light on this matter, in the present study we carried out a kinematic study aiming to assess whether and how visual and auditory properties of vitality forms expressed by others influenced the motor response of participants. In particular, participants were presented with video-clips showing a male and a female actor performing a “giving request” (give me) or a “taking request” (take it) in visual, auditory, and mixed modalities (visual and auditory). Most importantly, requests were expressed with rude or gentle vitality forms. After the actor's request, participants performed a subsequent action. Results showed that vitality forms expressed by the actors influenced the kinematic parameters of the participants' actions regardless to the modality by which they are conveyed. PMID:29204114

Models of energetic core-collapse supernovae and long-duration gamma-ray bursts often invoke engine-driven scenarios associated with the formation of compact objects that input energy into the explosion. To date, only indirect evidence of black holes or magnetars formed in these events exists from observations obtained when the explosions are most luminous. Here we request a modest 15 ks Chandra pilot observation of the exceptionally important nearby hypernova SN2002ap to test models that predict X-ray emission associated with its remnant black hole to be detectable after 15 yr of ejecta expansion. Direct observation a newly formed "baby" black hole would be a landmark discovery capable of opening up new ways to investigate fundamental aspects of the core collapse process.

The Aviation Data Integration System (ADIS) of Ames Research Center has been established to promote analysis of aviation data by airlines and other interested users for purposes of enhancing the quality (especially safety) of flight operations. The ADIS is a system of computer hardware and software for collecting, integrating, and disseminating aviation data pertaining to flights and specified flight events that involve one or more airline(s). The ADIS is secure in the sense that care is taken to ensure the integrity of sources of collected data and to verify the authorizations of requesters to receive data. Most importantly, the ADIS removes a disincentive to collection and exchange of useful data by providing for automatic removal of information that could be used to identify specific flights and crewmembers. Such information, denoted sensitive information, includes flight data (here signifying data collected by sensors aboard an aircraft during flight), weather data for a specified route on a specified date, date and time, and any other information traceable to a specific flight. The removal of information that could be used to perform such tracing is called "deidentification." Airlines are often reluctant to keep flight data in identifiable form because of concerns about loss of anonymity. Hence, one of the things needed to promote retention and analysis of aviation data is an automated means of de-identification of archived flight data to enable integration of flight data with non-flight aviation data while preserving anonymity. Preferably, such an automated means would enable end users of the data to continue to use pre-existing data-analysis software to identify anomalies in flight data without identifying a specific anomalous flight. It would then also be possible to perform statistical analyses of integrated data. These needs are satisfied by the ADIS, which enables an end user to request aviation data associated with de-identified flight data. The ADIS includes client software integrated with other software running on flight-operations quality-assurance (FOQA) computers for purposes of analyzing data to study specified types of events or exceedences (departures of flight parameters from normal ranges). In addition to ADIS client software, ADIS includes server hardware and software that provide services to the ADIS clients via the Internet (see figure). The ADIS server receives and integrates flight and non-flight data pertaining to flights from multiple sources. The server accepts data updates from authorized sources only and responds to requests from authorized users only. In order to satisfy security requirements established by the airlines, (1) an ADIS client must not be accessible from the Internet by an unauthorized user and (2) non-flight data as airport terminal information system (ATIS) and weather data must be displayed without any identifying flight information. ADIS hardware and software architecture as well as encryption and data display scheme are designed to meet these requirements. When a user requests one or more selected aviation data characteristics associated with an event (e.g., a collision, near miss, equipment malfunction, or exceedence), the ADIS client augments the request with date and time information from encrypted files and submits the augmented request to the server. Once the user s authorization has been verified, the server returns the requested information in de-identified form.

Using USNO's API to Obtain Data

The U.S. Naval Observatory (USNO) is in the process of modernizing its publicly available web services into APIs (Application Programming Interfaces). Services configured as APIs offer greater flexibility to the user and allow greater usage. Depending on the particular service, users who implement our APIs will receive either a PNG (Portable Network Graphics) image or data in JSON (JavaScript Object Notation) format. This raw data can then be embedded in third-party web sites or in apps.Part of the USNO's mission is to provide astronomical and timing data to government agencies and the general public. To this end, the USNO provides accurate computations of astronomical phenomena such as dates of lunar phases, rise and set times of the Moon and Sun, and lunar and solar eclipse times. Users who navigate to our web site and select one of our 18 services are prompted to complete a web form, specifying parameters such as date, time, location, and object. Many of our services work for years between 1700 and 2100, meaning that past, present, and future events can be computed. Upon form submission, our web server processes the request, computes the data, and outputs it to the user.Over recent years, the use of the web by the general public has vastly changed. In response to this, the USNO is modernizing its web-based data services. This includes making our computed data easier to embed within third-party web sites as well as more easily querying from apps running on tablets and smart phones. To facilitate this, the USNO has begun converting its services into APIs. In addition to the existing web forms for the various services, users are able to make direct URL requests that return either an image or numerical data.To date, four of our web services have been configured to run with APIs. Two are image-producing services: "Apparent Disk of a Solar System Object" and "Day and Night Across the Earth." Two API data services are "Complete Sun and Moon Data for One Day" and "Dates of Primary Phases of the Moon." Instructions for how to use our API services as well as examples of their use can be found on one of our explanatory web pages and will be discussed here.

Laboratory request forms provide information about the laboratory test being requested for. They carry demographic data and other information such as location of patient, laboratory number, doctor's name, signature of the doctor, telephone number of the requesting doctor. Omission of information on the forms may lead to laboratory errors. The aim of this study was to evaluate the level of completion of laboratory request forms at the haematology department of a Ghanaian tertiary hospital. Three thousand request forms submitted to the haematology department between January and April 2010 were retrieved and studied. The information provided on each request form was recorded in a spread sheet and analyzed. The patient's age and sex were missing in 25.6% and 32.7% of the forms respectively. About half of the request forms did not have the patient's location. No clinical detail was provided on 22.7% of the forms. Doctors were more likely to sign their request forms and provide a name but they all failed to provide an address or a contact telephone number. This study demonstrates that, the standard of completion of request forms was poor. Essential information required on the forms was often missing. This can lead to limited advice given by laboratory physicians and may increase the potential for errors. Conversely, provision of all the information needed on the forms will aid laboratory diagnosis and enhance patient care and save time and resources. There should be closer interaction between clinicians and laboratory personnel to improve quality of services.

Sidereal time and Julian date are two values often used in observational astronomy that can be tedious to calculate. Fortunately, the U.S. Naval Observatory (USNO) has redesigned its on-line Sidereal Time and Julian Date (JD) calculators to provide data through an Application Programming Interface (API). This flexible interface returns dates and times in JavaScript Object Notation (JSON) that can be incorporated into third-party websites or applications.Via the API, Sidereal Time can be obtained for any location on Earth for any date occurring in the current, previous, or subsequent year. Up to 9999 iterations of sidereal time data with intervals from 1 second to 1095 days can be generated, as long as the data doesn’t extend past the date limits. The API provides the Gregorian calendar date and time (in UT1), Greenwich Mean Sidereal Time, Greenwich Apparent Sidereal Time, Local Mean Sidereal Time, Local Apparent Sidereal Time, and the Equation of the Equinoxes.Julian Date can be converted to calendar date, either Julian or Gregorian as appropriate, for any date between JD 0 (January 1, 4713 BCE proleptic Julian) and JD 5373484 (December 31, 9999 CE Gregorian); the reverse calendar date to Julian Date conversion is also available.
