  1. Crashes involving motorised rickshaws in urban India: Characteristics and injury patterns

    Introduction Motorised three-wheeled vehicles (motorised rickshaw) are popular in Asian countries including India. This study aims to describe the crash characteristics and injury patterns for motorised rickshaw occupants and the road users hit-by-motorised rickshaw in urban India. Methods Consecutive cases of road traffic crashes involving motorised rickshaw, irrespective of injury severity, whether alive or dead, presenting to the emergency departments of two large government hospitals and three branches of a private hospital in Hyderabad city were recruited. Crash characteristics, details of injuries, injury severity parameters and outcome were documented in detailed interviews. Results A total of 139 (18%) of the 781 participants recruited were injured as a motorised rickshaw occupant (11%) or were hit by a motorised rickshaw (7%) in 114 crashes involving motorised rickshaw. Amongst motorised rickshaw occupants, single-vehicle collisions (54%) were more frequent than multi-vehicle collisions (46%), with overturning of motorised rickshaw in 73% of the single-vehicle collisions. Mortality (12%), the mean Injury Severity Score (5.8) and rate of multiple injured (60%) indicated a substantial trauma load. No significant differences in injury pattern were found between motorised rickshaw occupants and hit-by-motorised rickshaw subjects, with the pattern being similar to that of the pedestrians and two-wheeled vehicle users. With bivariate analysis for motorised rickshaw occupants, the risk of fatal outcome (odds ratio (OR) 2.60, 95% confidence interval (CI): 0.64–10.54), upper limb injury (OR 2.25, 95% CI: 0.94–5.37) and multiple injuries (OR 2.03, 95% CI 0.85–4.83) was high, although not statistically significant in multi-motorised-vehicle collisions as compared with the single-vehicle collisions or overturning. The risk of having multiple injuries (OR 4.55, 95% CI: 1.15–17.95) was significantly higher in motorised rickshaw occupants involved in front collisions. Being a front-seat motorised rickshaw passenger in a vehicle collision increased the risk of having a fatal outcome (OR 7.37, 95% CI: 0.83–65.66) and a Glasgow coma score ≤ 12 (OR 2.21, 95% CI: 0.49–9.89), although not significantly when compared to the back-seat passengers. Conclusion These findings can assist with planning to deal with the consequences and prevention of road traffic injuries due to crashes involving motorised rickshaw, given the high use of these and substantial morbidity of related injuries in India. The need for improved understanding of the risk characteristics of motorised rickshaw is highlighted. PMID:21584976

  2. Development of emission factors for motorcycles and shared auto-rickshaws using real-world driving cycle for a typical Indian city.

    Vehicular emission is one of the most important contributors of urban air pollution. To quantify the impact of traffic on urban air quality, it is necessary to quantify vehicular emission. In many cities of India, such as Dhanbad, shared auto-rickshaw is the pre-dominant mode of transportation. Indian Driving Cycle (IDC) and Modified Indian Driving Cycle (MIDC) are used for emission testing of motorcycles, shared auto-rickshaws and passenger cars in India for regulatory purposes. IDC used for motorcycles and shared auto-rickshaws does not recognize the difference in two vehicle classes in terms of driving pattern. In real world, shared auto-rickshaws, behave differently than motorcycles. To quantify the impact of shared auto-rickshaws on urban air quality accurately, emission factors (EFs) are required to derive from real-world driving cycles (DCs). In heterogeneous traffic, vehicles of one class affect the behavior of vehicles of other classes. To estimate the emissions from different vehicle classes accurately, EFs for motorcycles and passenger cars are also required to be revised. In this study, real-world DCs were developed for motorcycles, shared auto-rickshaws and passenger cars in Dhanbad. Developed DCs were used to calculate EFs for respective classes.   3. Health and Social Security Needs of Rickshaw Pullers in Ranchi.

    A significant number of "marginalized" population groups are denied their health rights, healthcare access, benefits of government health programs, and other health-related social security benefits. These population groups, particularly the rickshaw pullers of India, are vulnerable because of their poor social and economic status, migration history, lack of permanent settlements, lack of social capital, and so on. These reasons, coupled with other social-political reasons, lead to no or limited access to health services and poor health conditions. This study intends to explore and understand the context and health seeking behavior of rickshaw pullers in Ranchi, the capital city of Jharkhand state in India, with special reference to wider social determinants of health such as, access, affordability, and delivery of health services. The data was collected from 1,000 rickshaw pullers from 40 "rickshaw garages" in Ranchi. The findings of the study show that rickshaw pullers can't afford to "attach" much importance to their health due to various reasons and suffer from a variety of occupational and other illnesses. The health conditions of the rickshaw pullers are affected by poverty, negligence, illiteracy, lack of awareness and unavailability of affordable, quality health services. These are only a symptom of the larger structural issues affecting health. The study suggests that rickshaw pullers, being a marginalized and excluded population group, require special health and social security programs that include safe, affordable, and accessible services along with a special focus on health education, behavior change, and promotion of appropriate health-seeking behavior.

  4. High Prevalence of Chronic Respiratory Symptoms among Autorickshaw Drivers of Urban Puducherry, South India.

However, limited studies have determined the prevalence of chronic respiratory symptoms or chronic obstructive pulmonary disease (COPD) among drivers. Among auto-rickshaw drivers of urban Puducherry to determine prevalence of (i) chronic respiratory symptoms by using Indian Study on Epidemiology of Asthma, Respiratory Symptoms, and Chronic Bronchitis (INSEARCH) questionnaire and (ii) COPD by measuring peak expiratory flow rate (PEFR). Cross-sectional, descriptive study. Cluster random sampling was used to select 297 auto-rickshaw drivers. Subjects were interviewed using the INSEARCH questionnaire. PEFR was measured using Wright's peak flow meter. We also assessed exposure to tobacco smoke. Prevalence and 95% confidence interval of chronic respiratory symptoms and COPD were calculated. All the auto-rickshaw drivers were male, and 75% were in the age group of 31-50 years. They spend most of the time on the roadside either driving or waiting at the auto-stand. Prevalence of nonspecific respiratory symptoms among auto-rickshaw drivers was 76% (breathlessness on exertion 68%, cough at night and phlegm in the morning 22%, wheezing 18% assessed for last 12 months). Around 28% of them had PEFR <80%. The prevalence of tobacco smoking was also high (64%), and 100% had exposure to tobacco smoke at the workplace. The nonspecific chronic symptoms were high among auto-rickshaw drivers as compared to the general population noted from a multicentric study done in India. We recommend that auto-rickshaw drivers should use personal protective equipment and would require regular screening and treatment for respiratory impairment.

  6. Accidental cut-throat injuries from the broken windshield of an auto rickshaw: Two unusual cases.

    Accidental cut-throat injuries are extremely rare and usually involve a sharp-edged weapon. In this paper, two cases of a cut-throat wound to two auto-rickshaw drivers are presented where the broken windshield of the auto-rickshaws was responsible for the wounds. In both the cases, fatal incised wounds were present over the neck, cutting the soft tissue along with the major vessels. The death occurred due to exsanguination caused by neck-vessel injury in one case and trachea along with neck-vessel injury in the second case. Although the wounds on the neck initially suggested homicide, they were found to have occurred accidentally as a result of a road traffic accident involving a head-on collision of auto rickshaws. The injuries were inflicted by the shattered glass of the windshield.

  7. Evolving social health scheme for workers in unorganized sector: key evidences from study of cycle rickshaw pullers in Delhi, India.

    In view of high out-of-pocket costs and low spending even for basic healthcare for the poor employed in the unorganized sector, policy makers in India have turned their attention to developing a financing mechanism for social health insurance with the desire to provide quality care to the poor and economically disadvantaged. This study aims to assess and determine the disease profile, treatment expenditure and willingness to pay for health insurance among rickshaw pullers in Delhi. The study was conducted among 500 rickshaw pullers from five zones of the Municipal Corporation of Delhi, taking a sample of 100 from each zone. The average cost of treatment was Rs.505 for outpatient and Rs. 3200 for inpatient care. To finance the treatment expenditure, 27.5% of the respondents spent from their household savings, and 43% had to borrow funds. Any "spell of sickness" and "total expenditure on acute illness" were significantly (p < 0.01) associated with the willingness to pay for health insurance. Overall, the majority (83%) of participants were willing to pay for health insurance. The study provides the evidence for the need for urgent policy development by introducing a social health insurance package including wage losses for the vulnerable groups such as rickshaw pullers in the unorganized sector in India, which significantly contribute to pollution free and cheap transportation of community, tourists and commercial goods as well.   9. Pyrolysis kinetics behavior of solid tire wastes available in Bangladesh.

    Pyrolysis kinetics of available bicycle/rickshaw, motorcycle and truck tire wastes in Bangladesh have been investigated thermogravimetrically in a nitrogen atmosphere at heating rates of 10 and 60 degrees C/min over a temperature range of 30-800 degrees C. The three tire wastes exhibited similar behaviors in that, when heating rate was increased, the initial reaction temperature decreased but the reaction range and reaction rate increased. The percentage of total weight loss was higher for truck tire waste and lower for bicycle/rickshaw tire waste. The pyrolysis of truck tire waste was found to be easier than that of bicycle/rickshaw and motorcycle tire wastes while it was comparatively more difficult for motorcycle tire waste. The overall rate equation for the three tire wastes has been modeled satisfactorily by one simplified equation from which the kinetic parameters of unreacted materials based on the Arrhenius form can be determined. The predicted rate equation compares fairly well with the measured TG and DTG data. DTA curves for all of the samples show that the degradation reactions are three main exotherms and one endotherm.

    Vehicular air pollution is a mounting health issue of the modern age, particularly in urban populations of the developing nations. Auto-rickshaws are not considered eco-friendly as to their inefficient engines producing large amount of particulate matter (PM), thus posing significant environmental threat. The present study was conducted to ascertain the cytotoxic, phytotoxic, and mutagenic potential of PM from gasoline-powered two-stroke auto-rickshaws (TSA) and compressed natural gas-powered four-stroke auto-rickshaws (FSA). Based on the increased amount of aluminum quantified during proton-induced X-ray emission analysis of PM from TSA and FSA, different concentrations of aluminum sulfate were also tested to determine its eco-toxicological potential. The MTT assay demonstrated significant (p < 0.001) dose-dependent cytotoxic effects of different concentrations of TSA, FSA, and aluminum sulfate on BHK-21 cell line. LC50 of TSA, FSA, and aluminum sulfate was quantified at 16, 11, and 23.8 μg/ml, respectively, establishing PM from FSA, a highly cytotoxic material. In case of phytotoxicity screening using Zea mays, the results demonstrated that all three tested materials were equally phytotoxic at higher concentrations producing significant reduction (p < 0.001) in seed germination. Aluminum sulfate proved to be a highly phytotoxic agent even at its lowest concentration. Mutagenicity was assessed by fluctuation Salmonella reverse mutation assay adopting TA100 and TA98 mutant strains with (+S9) and without (-S9) metabolic activation. Despite the fact that different concentrations of PM from both sources, i.e., TSA and FSA were highly mutagenic (p < 0.001) even at lower concentrations, the mutagenic index was higher in TSA. Data advocate that all tested materials are equally ecotoxic, and if the existing trend of atmospheric pollution by auto-rickshaws is continued, airborne heavy metals will seriously affect the normal growth of local inhabitants and increased contamination of agricultural products, which will amplify the dietary intake of the toxic elements and could result in genetic mutation or long-term health implications.

  11. Pollution dilemma in Asian population: CNG and wound healing.

    Automobile exhaust constituents contribute significantly to air pollution in urban areas and compressed natural gas (CNG) is considered one of the most promising fuel alternatives for the future. CNG-powered four-stroke engine auto-rickshaws are ubiquitous in South Asian cities as taxi and for commercial transportation. Automotive exhaust contains several toxins, which are overwhelmingly toxic to the processes of wound healing. By utilizing the in vivo mouse model of wound healing, this report analyzes the effects of CNG-powered four-stroke auto-rickshaws smoke solution (4SARSS) on different events of wound healing; dermal matrix regeneration, re-epithelialization and neovascularization. A total of 72 adult mice, divided in eight groups were exposed to 4SARSS for 12 days. A highly significant reduction (P<0.001) in wound closure was observed among all 4SARSS treated groups, at each time point of the experiment. An immature development in both the neoepidermis and the neodermis was observed among all 4SARSS treated wounds with defective re-epithelialization, dermal matrix regeneration and maturation of collagen bundles. Abbott curve, angular spectrum, 3D surface topographies, and histological investigations of wounds explicated highly significant activation (P<0.001) of delayed-neovascularization among 4SARSS treated wounds. All these annotations advocate excessive toxicity of emission from CNG-powered auto-rickshaws to the process of wound healing and people occupationally exposed to this toxic emissions may suffer varying degree of delayed wound healing.     Objectives: The informal sector is the dominant area of employment and the economy for any developing country including Bangladesh. The cost of productivity loss due to absence from work or presenteeism with illness has rarely been examined in the Bangladesh context. This current study, therefore, attempted to examine the impact of ill health of informal sector workers on labor productivity, future earning, and healthcare-related expenditure. Methodology: A cross-sectional survey was conducted among three occupational groups of informal workers (rickshaw pullers, shopkeepers and restaurant workers) that were generally found in all urban areas in Bangladesh. A total of 557 informal workers were surveyed for this study. Results: Most of the respondents (57%) reported that they had been affected by some type of illness for the last six months. The overall average healthcare expenditure of informal workers was US$48.34, while restaurant workers expended more (US$53.61). Self reported sickness absenteeism was highest (50.37days) in the case of shop keepers, followed by rickshaw pullers (49.31 days), in the last six months. Considering the income loss due to illness in the past six months, the rickshaw pullers were exposed to the highest income loss (US$197.15), followed by the shop keepers (US$151.39). Conclusions: Although the informal sector contributes the most to the economy of Bangladesh, the workers in this sector have hardly any financial protection. This study provides critical clues to providing financial and social protection to informal sector workers in Bangladesh. PMID:27010089

  14. Common mental disorders in public transportation drivers in Lima, Peru.

    Traffic related injuries are leading contributors to burden of disease worldwide. In developing countries a high proportion of them can be attributed to public transportation vehicles. Several mental disorders including alcohol and drug abuse, psychotic disorders, mental stress, productivity pressure, and low monetary income were found predictors of high rates of traffic related injuries in public transportation drivers. The goal of this study was to estimate the prevalence of common mental disorders in the population of public transportation drivers of buses and rickshaws in Lima, Peru. Cross sectional study. A sample of bus and rickshaw drivers was systematically selected from formal public transportation companies using a snowball approach. Participants completed self-administered questionnaires for assessing major depressive episode, anxiety symptoms, alcohol abuse, and burnout syndrome. Socio demographic information was also collected. The analyses consisted of descriptive measurement of outcomes taking into account both between and within cluster standard deviation (BCSD and WCSD). A total of 278 bus and 227 rickshaw drivers out of 25 companies agreed to participate in the study. BCSD for major depressive episode, anxiety symptoms and burnout syndrome was not found significant (p>0.05). The estimated prevalence of each variable was 13.7% (IC95%: 10.7-16.6%), 24.1% (IC95%: 19.4-28.8%) and 14.1% (IC95%: 10.8-17.4%) respectively. The estimated prevalence of alcohol abuse was 75.4% (IC95%: 69-81.7%, BCSD = 12.2%, WCSD = 41.9%, intra class correlation (ICC): 7.8%). Common mental disorders such as alcohol abuse, major depressive episode, anxiety symptoms and burnout syndrome presented higher rates in public transportation drivers than general population.

  15. Concentrations of fine, ultrafine, and black carbon particles in auto-rickshaws in New Delhi, India

    Concentrations of air pollutants from vehicles are elevated along roadways, indicating that human exposure in transportation microenvironments may not be adequately characterized by centrally located monitors. We report results from ˜180 h of real-time measurements of fine particle and black carbon mass concentration (PM 2.5, BC) and ultrafine particle number concentration (PN) inside a common vehicle, the auto-rickshaw, in New Delhi, India. Measured exposure concentrations are much higher in this study (geometric mean for ˜60 trip-averaged concentrations: 190 μg m -3 PM 2.5, 42 μg m -3 BC, 280 × 10 3 particles cm -3; GSD ˜1.3 for all three pollutants) than reported for transportation microenvironments in other megacities. In-vehicle concentrations exceeded simultaneously measured ambient levels by 1.5× for PM 2.5, 3.6× for BC, and 8.4× for PN. Short-duration peak concentrations (averaging time: 10 s), attributable to exhaust plumes of nearby vehicles, were greater than 300 μg m -3 for PM 2.5, 85 μg m -3 for BC, and 650 × 10 3 particles cm -3 for PN. The incremental increase of within-vehicle concentration above ambient levels—which we attribute to in- and near-roadway emission sources—accounted for 30%, 68% and 86% of time-averaged in-vehicle PM 2.5, BC and PN concentrations, respectively. Based on these results, we estimate that one's exposure during a daily commute by auto-rickshaw in Delhi is as least as large as full-day exposures experienced by urban residents of many high-income countries. This study illuminates an environmental health concern that may be common in many populous, low-income cities.

    A public health concern is to understand the linkages between specific pollution sources and adverse health impacts. Commuting can be viewed as one of the significant-exposure activity in high-vehicle density areas. This paper investigates the commuter exposure to inhalable, thoracic and alveolic particles in various transportation modes in Delhi, India. Air pollution levels are significantly contributed by automobile exhaust and also in-vehicle exposure can be higher sometime than ambient levels. Motorcycle, auto rickshaw, car and bus were selected to study particles concentration along two routes in Delhi between Kashmere Gate and Dwarka. The bus and auto rickshaw were running on compressed natural gas (CNG) while the car and motorcycle were operated on gasoline fuel. Aerosol spectrometer was employed to measure inhalable, thoracic and alveolic particles during morning and evening rush hours for five weekdays. From the study, we observed that the concentration levels of these particles were greatly influenced by transportation modes. Concentrations of inhalable particles were found higher during morning in auto rickshaw (332.81 ± 90.97 μg/m(3)) while the commuter of bus exhibited higher exposure of thoracic particles (292.23 ± 110.45 μg/m(3)) and car commuters were exposed to maximum concentrations of alveolic particles (222.37 ± 26.56 μg/m(3)). We observed that in evening car commuters experienced maximum concentrations of all sizes of particles among the four commuting modes. Interestingly, motorcycle commuters were exposed to lower levels of inhalable and thoracic particles during morning and evening hours as compared to other modes of transport. The mean values were found greater than the median values for all the modes of transport suggesting that positive skewed distributions are characteristics of naturally occurring phenomenon.     The road traffic systems, travel patterns and other traffic characteristics are different for each country due to differences in the geometric patterns, available transport facilities for commuters, proportional and type of the vehicle itself and so on. In Indonesia, the standard of pce (Passenger Car Equivalent) value found on IHCM (Indonesian Highway Capacity Manual) published in 1997. IHCM stated that the value of pce for heavy vehicles and motorcycles are 1.3 and 0.5 respectively. On these day, regarding Medan as a third biggest city in Indonesia, there have been lot of changes with regarding to the composition of the vehicle, as well as variations of the type of the vehicle itself. Becak bermotor (motorized tricycles) is a vehicle which is widely available in the city of Medan. Data from Medan City Transportation Department stated that there are more than 20,000 motorized motorized tricycles vehicles operating in the city at these day. Pce value of these rickshaws will be calculated based on observations at road and intersections in Medan. The calculation result shows that the pce value of motorized rickshaw is more than 1. This value will make the calculations regarding the performance of the traffic can be performed more accurately.

    In south Asian countries, a campaign has launched to promote CNG-powered four-stroke auto-rickshaws (CNFAR) to decrease emission load in the environment. Even though, CNFAR are considered environmentally safe, emissions of some other toxic chemicals would amplify, which may effect the development of growing fetus and may result in different growth defects. By utilizing the in vivo chicken embryo model, this report analyzes the toxic potential of CNFAR smoke solutions (CNFARSS) on embryonic movements (EM) and cardiovascular development. Application of CNFARSS to embryos caused profound decline (p<0.001) in all four types of EMs. Several recovery attempts of all EMs were observed in oscillating fashion, however, EMs did not recover by the end of experiment. Exposure of CNFARSS escorted intense decline (p<0.001) with temperate recovery phases in the EM of tail. Macroscopic evaluation of all CNFARSS treated chicken embryos revealed several widespread hemorrhaging throughout the whole body. Moreover, four different types of ectopia cordis were prominently observed among all CNFARSS treated embryos, namely; incomplete ectopia cordis, complete ectopia cordis, cervico-thoracic ectopia cordis and thoraco-abdominal ectopia cordis.

    Major transformations in the environmental composition are principally attributable to the combustion of fuels by automobiles. Motorized gasoline-powered two-stroke auto-rickshaws (TSA) and compressed natural gas (CNG)-powered four-stroke auto-rickshaws (FSA) are potential source of air pollution in south Asia and produce toxic amount of particulate matter (PM) to the environment. In this study, we attempted to characterize elemental pollutants from the PM of TSA and FSA using proton-induced X-ray emission (PIXE) analysis. The observations of the existing investigation recognized significant increase in Al (P < 0.05), P (P < 0.01), and Zn (P < 0.01) from the PM samples of FSA. In addition, the concentrations of Cu, Fe, K, Mg, Na and S were also observed exceeding the recommended National Institute for Environmental Studies limits. On the contrary, increased concentration of Sr and V were observed in the PM samples from TSA. It is generally believed that FSA generates smaller amount of PM but data obtained from FSA are clearly describing that emissions from FSA comprised potentially more toxic substances than TSA. The current research is specific to metropolitan population and has evidently revealed an inconsistent burden of exposure to air pollutants engendered by FSA in urban communities, which could lead to the disruption of several biological activities and may cause severe damage to entire ecological system.

    Auto-rickshaws in India use different fuels and engine technologies, with varying emissions and implications for air quality and climate change. Chassis dynamometer emission testing was conducted on 30 in-use auto-rickshaws to quantify the impact of switching from gasoline to compressed natural gas (CNG) in spark-ignition engines. Thirteen test vehicles had two-stroke CNG engines (CNG-2S) and 17 had four-stroke CNG engines (CNG-4S), of which 11 were dual-fuel and operable on a back-up gasoline (petrol) system (PET-4S). Fuel-based emission factors were determined for gaseous pollutants (CO(2), CH(4), NO(X), THC, and CO) and fine particulate matter (PM(2.5)). Intervehicle variability was high, and for most pollutants there was no significant difference (95% confidence level) between "old" (1998-2001) and "new" (2007-2009) age-groups within a given fuel-technology class. Mean fuel-based PM(2.5) emission factor (mean (95% confidence interval)) for CNG-2S (14.2 g kg(-1) (6.2-26.7)) was almost 30 times higher than for CNG-4S (0.5 g kg(-1) (0.3-0.9)) and 12 times higher than for PET-4S (1.2 g kg(-1) (0.8-1.7)). Global warming commitment associated with emissions from CNG-2S was more than twice that from CNG-4S or PET-4S, due mostly to CH(4) emissions. Comprehensive measurements and data should drive policy interventions rather than assumptions about the impacts of clean fuels.

  5. On-road PM2.5 pollution exposure in multiple transport microenvironments in Delhi

    PM2.5 pollution in Delhi averaged 150 μg/m3 from 2012 through 2014, which is 15 times higher than the World Health Organization's annual-average guideline. For this setting, we present on-road exposure of PM2.5 concentrations for 11 transport microenvironments along a fixed 8.3-km arterial route, during morning rush hour. The data collection was carried out using a portable TSI DustTrak DRX 8433 aerosol monitor, between January and May (2014). The monthly-average measured ambient concentrations varied from 130 μg/m3 to 250 μg/m3. The on-road PM2.5 concentrations exceeded the ambient measurements by an average of 40% for walking, 10% for cycle, 30% for motorised two wheeler (2W), 30% for open-windowed (OW) car, 30% for auto rickshaw, 20% for air-conditioned as well as for OW bus, 20% for bus stop, and 30% for underground metro station. On the other hand, concentrations were lower by 50% inside air-conditioned (AC) car and 20% inside the metro rail carriage. We find that the percent exceedance for open modes (cycle, auto rickshaw, 2W, OW car, and OW bus) reduces non-linearly with increasing ambient concentration. The reduction is steeper at concentrations lower than 150 μg/m3 than at higher concentrations. After accounting for air inhalation rate and speed of travel, PM2.5 mass uptake per kilometer during cycling is 9 times of AC car, the mode with the lowest exposure. At current level of concentrations, an hour of cycling in Delhi during morning rush-hour period results in PM2.5 dose which is 40% higher than an entire-day dose in cities like Tokyo, London, and New York, where ambient concentrations range from 10 to 20 μg/m3.

The bidding game version of contingent valuation method was used to estimate weekly WTP. In three urban locations 557 workers were interviewed using a structured questionnaire during 2010 and 2011. Multiple-regression analysis was used to predict WTP by demographic and household characteristics, occupation, education level and past illness. WTP for a CBHI scheme was expressed by 86.7% of informal workers. Weekly average WTP was 22.8 BDT [Bangladeshi Taka; 95% confidence interval (CI) 20.9-24.8] or 0.32 USD and varied significantly across occupational groups (p = 0.000) and locations (p = 0.003). WTP was highest among rickshaw-pullers (28.2 BDT or 0.40 USD; 95% CI: 24.7-31.7), followed by restaurant workers (20.4 BDT 0.29 USD; 95% CI: 17.0-23.8) and shopkeepers (19.2 BDT or 0.27 USD; 95% CI: 16.1-22.4). Multiple regression analysis identified monthly income, occupation, geographical location and educational level as the key determinants of WTP. WTP increased 0.196% with each 1% increase in monthly income, and was 26.9% lower among workers with up to a primary level of education versus those with higher than primary, but less than one year of education. Informal workers in urban areas thus are willing to pay for CBHI and socioeconomic differences explain the magnitude of WTP. The policy maker might think introducing community-based model including public-community partnership model for healthcare financing of informal workers. Decision making regarding the implementation of such schemes should consider worker location and occupation.

    The EMBL-Hamburg Automated Crystal Structure Determination Platform is a system that combines a number of existing macromolecular crystallographic computer programs and several decision-makers into a software pipeline for automated and efficient crystal structure determination. The pipeline can be invoked as soon as X-ray data from derivatized protein crystals have been collected and processed. It is controlled by a web-based graphical user interface for data and parameter input, and for monitoring the progress of structure determination. A large number of possible structure-solution paths are encoded in the system and the optimal path is selected by the decision-makers as the structure solution evolves. The processes have been optimized for speed so that the pipeline can be used effectively for validating the X-ray experiment at a synchrotron beamline.

  9. Road rage and road traffic accidents among commercial vehicle drivers in Lahore, Pakistan.

    Road rage and road traffic accidents increase the burden of morbidity and mortality in a population. A cross-sectional survey with convenience sampling was conducted among commercial vehicle drivers in Lahore, Pakistan (n = 901) to record their behaviours/experiences regarding road rage and road traffic accidents. Respondents were asked about incidents of shouting/cursing/rude gestures or threats to physically hurt the person/vehicle, by others or themselves, in the previous 24 hours or 3 months, and their involvement in road traffic accidents in the previous 12 months. Auto-rickshaw drivers were significantly more likely to report various road rage experiences/behaviours and involvement in accidents compared with bus and wagon drivers. A total of 112 respondents (12.4%) reported being involved in a road traffic accident in the previous 12 months but traffic police did not record the accident in 52.7% of cases. The results of this study underline the need to improve road safety in Pakistan.

    Occupational exposure to air pollution induces oxidative stress and prolonged exposure increases susceptibility to cardiovascular and respiratory diseases in several working groups. Biofluid of these subjects may reflect perturbed metabolic phenotypes. In this study we carried out a comparative molecular profiling study using parallel biofluids collected from subjects (n = 85) belonging to auto rickshaw drivers (ARD), traffic cops (TC) and office workers (OW). Higher levels of oxidative stress and inflammation markers in serum of ARD subjects were observed as compared to OW and TC. Uni and multivariate analyses of metabolites identified in urine by 1H NMR revealed 11 deregulated molecules in ARD subjects and involved in phenylalanine, histidine, arginine and proline metabolism. Despite contribution of confounding factors like exposure period, dietary factors including smoking and alcohol status, our results demonstrate existence of exposure specific metabotypes in biofluids of ARD, OW and TC groups. Monitoring serum oxidative stress and inflammation markers and urine metabolites by NMR may be useful to characterize perturbed metabolic phenotypes in populations exposed to urban traffic air pollution.

Methods: A survey was conducted to observe the present scenarios of secondary disposal site (SDS), ultimate disposal site (UDS), composting plants, medical wastes management and NGOs and CBOs MSW management activities. Results: A total of 22 NGOs and CBOs are involved in MSW management in 31 wards of Khulna City Corporation. About 9 to 12% of total generated wastes are collected by door-to-door collection system provided by mainly NGOs and CBOs using 71 non-motorized rickshaw vans. A major portion of collected wastes is disposed to the nearest SDS by these organizations and then transferred to UDS or to private low-lying lands from there by the city authority. A small portion of organic wastes is going to the composting plants of NGOs. Conclusion: The participation of NGOs and CBOs has improved the overall MSW management system, especially waste collection process from sources and able to motivate the residents to store the waste properly and to keep clean the premises.   13. Evolution of on-road vehicle exhaust emissions in Delhi

    For a 40-year horizon (1990-2030), on-road vehicle exhaust emissions were evaluated, retrospectively and prospectively, for the largest urban agglomeration in India - the Greater Delhi region with a combined population of 22 million in 2011 (Delhi along with Ghaziabad, Noida, Greater Noida, Faridabad and Gurgaon). Emissions of particulate matter, sulfur dioxide, carbon monoxide and volatile organic compounds (VOCs) reached their peak during late 1990s through early 2000s after which they reduced significantly through year 2012. On the other hand, nitrogen oxides (NOx) and carbon dioxide show an increasing trend. The most reduction in emissions between 1998 and 2012 occurred as a result of implementation of four sets of vehicular emission standards, removal of lead, reduction of sulfur content, mandatory retirement of older commercial vehicles, and conversion of diesel and petrol run public transport vehicles to compressed natural gas. In addition, changes in the vehicular technology have also contributed to controlling emissions especially in case of auto-rickshaws and motorized two-wheelers, which changed from two-stroke to four-stroke. The rising trend of NOx along with the presence of VOCs indicates increasing tendency to form ground-level ozone and as a result, smog in the region. We predict that the current regime of vehicle technology, fuel standards, and high growth rate of private vehicles, is likely to nullify all the past emission reductions by the end of 2020s.

    On road transport emissions is kicking off in Indian cities due to high levels of urbanization and economic growth during the last decade in Indian subcontinent. In 1951, about 17% of India's population were living in urban areas that increased to 32% in 2011. Currently, India is fourth largest Green House Gas (GHG) emitter in the world, with its transport sector being the second largest contributor of CO2 emissions. For achieving prospective carbon reduction targets, substantial opportunity among in-use vehicle is necessary to quantify. Since, urban traffic flow and operating condition has significant impact on exhaust emission (Choudhary and Gokhale, 2016). This study examined the influence of vehicular operating kinetics on CO2 emission from predominant private transportation vehicles of Indian metropolitan city, Guwahati. On-board instantaneous data were used to quantify the impact of CO2 emission on different mileage passenger cars and auto-rickshaws at different times of the day. Further study investigates CO2 emission reduction strategies by using International Vehicle Emission (IVE) model to improve co-benefit in private transportation by integrated effort such as gradual phase-out of inefficient vehicle and low carbon fuel. The analysis suggests that fuel type, vehicles maintenance and traffic flow management have potential for reduction of urban sector GHG emissions.     This paper examines the loan-use pattern of women involved in wage employment and their benefits from such loans in Bangladesh. The effects of wage employment on gender relations and how these women balance loan use, wage employment, and housework were also explored. The study was conducted among women enrolled in the Ayesha Abed Foundation (AAF) of the Bangladesh Rural Advancement Committee (BRAC) in Jamalpur district, central Bangladesh. The AAF was established to generate employment and income for poor rural women. Data were gathered through survey, interviews, and focus-group discussions. Findings revealed that women wage earners avail themselves of the BRAC loans for consumption, asset accumulation, land purchase, and other productive purposes. About 53% of their loans were used by others and only 34 out of 341 women in the sample actually used the loans themselves. The loans were repaid by sewing or subsistence work, mainly in the subcenters (52%), through income from rickshaw pulling by their husbands (24%), and by selling vegetables, eggs, or milk. Furthermore, findings showed that the household work of women wage earners is generally taken up by other women in the family and has resulted in more men taking part in household responsibilities. In conclusion, wage employment plays an important factor in the promotion of the economic and social empowerment of women. Economic empowerment is observed in the greater degree of control women have over the money they earn. Social empowerment is manifested in the expanding mobility of women, whereby they are able to interact with other women and generate support systems.

  16. Modelling of road traffic fatalities in India.

    Passenger modes in India include walking, cycling, buses, trains, intermediate public transport modes (IPT) such as three-wheeled auto rickshaws or tuk-tuks, motorised two-wheelers (2W) as well as cars. However, epidemiological studies of traffic crashes in India have been limited in their approach to account for the exposure of these road users. In 2011, for the first time, census in India reported travel distance and mode of travel for workers. A Poisson-lognormal mixture regression model is developed at the state level to explore the relationship of road deaths of all the road users with commute travel distance by different on-road modes. The model controlled for diesel consumption (proxy for freight traffic), length of national highways, proportion of population in urban areas, and built-up population density. The results show that walking, cycling and, interestingly, IPT are associated with lower risk of road deaths, while 2W, car and bus are associated with higher risk. Promotion of IPT has twofold benefits of increasing safety as well as providing a sustainable mode of transport. The mode shift scenarios show that, for similar mode shift across the states, the resulting trends in road deaths are highly dependent on the baseline mode shares. The most worrying trend is the steep growth of death burden resulting from mode shift of walking and cycling to 2W. While the paper illustrates a limited set of mode shift scenarios involving two modes at a time, the model can be applied to assess safety impacts resulting from a more complex set of scenarios.   17. Epidemiology of road traffic injury patients presenting to a tertiary hospital in Hyderabad, India.

    Road traffic injuries kill more people in India than in any other country in the world, and these numbers are rising with increasing population density and motorization. Official statistics regarding road traffic injuries are likely subject to underreporting. This study presents results of a surveillance program based at a public tertiary hospital in Hyderabad, India. All consenting patients who presented to the casualty ward after a road traffic injury over a 9-month period were enrolled. Interviews were performed and data abstracted from clinical records by trained research assistants. Data included demographics, injury characteristics, risk factors, safety behaviors, and outcomes. A total of 5,298 patients were enrolled; their mean age was 32.4 years (standard deviation 13.8) and 87.3% were men; 58.2% of patients were injured while riding a motorcycle or scooter, 22.5% were pedestrians, and 9.2% used motorized rickshaws. The most frequent collision type was skid or rollover (40.9%). Male victims were younger than female victims and were overrepresented among motorized 2-wheeler users. Patients were most frequently injured from 1600 to 2400. A total of 27.3% of patients were admitted. Hospital mortality was 5.3%, and 48.2% of deaths were among motorized 2-wheeler users. This is one of the few prospective, hospital-based studies of road traffic injury epidemiology in India. The patient population in this study was similar to prior hospital-based studies. When compared to government surveillance systems, this study showed motorized 2-wheeler users to be more frequently represented among the overall population and among fatalities.   18. A hybrid model for predicting carbon monoxide from vehicular exhausts in urban environments

    Several deterministic-based air quality models evaluate and predict the frequently occurring pollutant concentration well but, in general, are incapable of predicting the 'extreme' concentrations. In contrast, the statistical distribution models overcome the above limitation of the deterministic models and predict the 'extreme' concentrations. However, the environmental damages are caused by both extremes as well as by the sustained average concentration of pollutants. Hence, the model should predict not only 'extreme' ranges but also the 'middle' ranges of pollutant concentrations, i.e. the entire range. Hybrid modelling is one of the techniques that estimates/predicts the 'entire range' of the distribution of pollutant concentrations by combining the deterministic based models with suitable statistical distribution models ( Jakeman, et al., 1988). In the present paper, a hybrid model has been developed to predict the carbon monoxide (CO) concentration distributions at one of the traffic intersections, Income Tax Office (ITO), in the Delhi city, where the traffic is heterogeneous in nature and meteorology is 'tropical'. The model combines the general finite line source model (GFLSM) as its deterministic, and log logistic distribution (LLD) model, as its statistical components. The hybrid (GFLSM-LLD) model is then applied at the ITO intersection. The results show that the hybrid model predictions match with that of the observed CO concentration data within the 5-99 percentiles range. The model is further validated at different street location, i.e. Sirifort roadway. The validation results show that the model predicts CO concentrations fairly well ( d=0.91) in 10-95 percentiles range. The regulatory compliance is also developed to estimate the probability of exceedance of hourly CO concentration beyond the National Ambient Air Quality Standards (NAAQS) of India. It consists of light vehicles, heavy vehicles, three- wheelers (auto rickshaws) and two-wheelers (scooters, motorcycles, etc).

  19. Environmental monitoring of adulterated gasoline with kerosene and their assessment at exhaust level.

    Fuel adulteration increases the emission of total hydrocarbons, carbon monoxide, nitrogen oxides and respirable particulate matter, and thus adds to air pollution. The study examined the effects of mixing of different percentage of kerosene with petrol on the motorized rickshaw exhausts in terms of volatile organic compounds (benzene, toluene, xylene and ethyl benzene) and total suspended particulate matter (SPM). The personal sampler was used for sampling, and gas chromatography-mass spectrometry for quantification of compounds. Concentration of volatile organic compounds significantly decreased (p < 0.001) along with the increase in fraction of kerosene in petrol. The level of benzene in exhausts while, using petrol (100%) was significantly higher (p < 0.001) than that of three combinations used in this study (75% petrol + 25% kerosene, 50% petrol + 50% kerosene and 25% petrol + 75% kerosene). Similar trend was observed for toluene, xylene and ethyl benzene also. The mean concentration of benzene, toluene, xylene and ethyl benzene were 31.34,160.93, 10.07 and 5.58 microg m(-3) in pure petrol, while 12.30, 51.41,4.89 and 3.16 microg m(-3) for fuel combination 75% petrol + 25% kerosene. The observed levels of benzene, toluene, xylene and ethyl benzene were 9.12, 41.04, 4.33 and 2.91 microg m(-3) for fuel mixture having 50% petrol with 50% kerosene and levels were 8.36, 20.05, 3.82 and 2.95 microg m(-3) were for 25% petrol with 75% kerosene fuel combination. The levels of suspended particulate matter (SPM) increased along with the increase in fraction of kerosene in petrol. The data generated is useful to understand the common volatile organic compounds trend with the increasing fraction of kerosene in petrol.

  20. Comparison of radiofrequency electromagnetic field exposure levels in different everyday microenvironments in an international context.

    The aim of this study was to quantify RF-EMF exposure applying a tested protocol of RF-EMF exposure measurements using portable devices with a high sampling rate in different microenvironments of Switzerland, Ethiopia, Nepal, South Africa, Australia and the United States of America. We used portable measurement devices for assessing RF-EMF exposure in 94 outdoor microenvironments and 18 public transport vehicles. The measurements were taken either by walking with a backpack with the devices at the height of the head and a distance of 20-30 cm from the body, or driving a car with the devices mounted on its roof, which was 170-180 cm above the ground. The measurements were taken for about 30 min while walking and about 15-20 min while driving in each microenvironment, with a sampling rate of once every 4 s (ExpoM-RF) and 5 s (EME Spy 201). Mean total RF-EMF exposure in various outdoor microenvironments varied between 0.23 V/m (non-central residential area in Switzerland) and 1.85 V/m (university area in Australia), and across modes of public transport between 0.32 V/m (bus in rural area in Switzerland) and 0.86 V/m (Auto rickshaw in urban area in Nepal). For most outdoor areas the major exposure contribution was from mobile phone base stations. Otherwise broadcasting was dominant. Uplink from mobile phone handsets was generally very small, except in Swiss trains and some Swiss buses. This study demonstrates high RF-EMF variability between the 94 selected microenvironments from all over the world. Exposure levels tended to increase with increasing urbanity.
